Janssen Vaccine was given to this patient who was 12 years old. He was monitored after vaccine for 15 minutes with no problems. At the end of the shift we realized that the vaccine was given to someone under the age of 18. Dr. who was on duty called and spoke to patients mother. She stated patient was doing good. She did say he had complained of mild headache but that was it. He offered patients mother to bring him in for a recheck but the mother felt he was fine and would notify us if there were any concerns.

Pediatric patient presented to Covid vaccination clinic for scheduled first dose of Pfizer. Inadvertently patient received 0.3ml of Janssen vaccine instead of 0.3 ml of Pfizer vaccine. Patient had headache and tiredness in the days following vaccination was fine on 6/23.

Patient and mom attended school vaccine event on 6/8. Mom requested patient to receive Janssen vaccine. Nursing staff did not redirect to Pfizer based on patient age and administered the Janssen vaccine. Patient waited 15 min of observation and then left with mom. Later in the evening when paperwork was being sorted, the error was discovered. As the supervisor of the team I was notified. I called and spoke with mom around 7pm. No side effects or issues with the patient currently. Instructed to watch him overnight and I would follow up in the morning. This morning I spoke with mom and learned that at 2300 patient complained of light headedness and body aches. At 0345 woke up with a headache that was treated

13 year old received the Jansen vaccine. Spoke to parent on 7/19/2021. Parent stated that he had a fever of 102 degree Fahrenheit for about 6 hours and felt achy for about 24 hours. She stated he was doing fine and had been doing yard work. Spoke to parent on 7/23/21. I informed her that he did not need an additional vaccine. She thanked me for calling. She said he was still doing fine and was currently away at camp.

Patient has had fever, headache, nausea, vomiting, muscles aches, fatigue and diarrhea off and on since vaccination. On 6/24/21 at 12:30AM went to ER due to headache, vomiting, diarrhea and weakness. Diagnosed with a bacterial infection in his stool. Given medicine for pain and vomiting. Per mom, all tests done including for concerns with blood, heart and brain were all normal.

Reporting to VAERS due to patient under 18 years of age. Patient had fever the night of administration, felt weak ("similar to flu-like symptoms") the next day and all symptoms resolved within 24 hours.
