COVID infection post vaccination 1 day following vaccination, patient experienced fatigue, myalgia and low grade fever. RA test positive. That evening, patient with increasing SOB and increasing oxygen requirement and was admitted for hypoxia + LE swelling. Although SNF where vaccination took place denied symptoms at time of vaccination, per H&P, increasing oxygen requirements were noted 3 days prior to vaccination. See H&P below He is currently residing in a rehab facility following a hospitalization detailed below, but essentially he noted some shortness of breath about 4-5 days ago. They checked his oxygen saturation and noted that he was hypoxic, so started him on oxygen (reportedly 4L NC?). He reports a worsened productive cough; sputum is dark but not bloody in color. He has been fatigued and chilled; he thinks he's been intermittently febrile. No nausea, vomiting, or constipation. He endorses twice daily diarrhea, but he associates this with his bowel regimen. He also has a fair amount of lower extremity swelling which is bothersome to him; he says this has been ongoing since his MVA in January.[KE.4] He was recently seen in Nephrology clinic, where he was started on lasix 40mg daily and started on a low sodium diet[KE.3]. He is unsure if the lasix is helping his swelling, but does have robust urine output after he takes it

I had a residual cough when I had the vaccine. Up to a week after vaccine, I had low grade fever and chills; night sweating and coughing seemed to be getting worse and coughing getting more frequent and worse (when it had been dry and occasional). It was ongoing for a week. I called my Doctor during that time and she prescribed me medications including an antibiotic and cold medications for the cough. The medications helped symptoms after I'd been on them awhile . I no longer have the fever and I stopped the night sweating. Cough seems to be improved.

Patient stated he woke up with red welts on his left arm and experienced numbness/swelling on the left side of his face. The patient compared the numbness to the effects of novocaine at the dentist. The patient also experienced fever, chills and sweating. All of the symptoms resolved within a few hours and the patient was feeling better when we spoke later in the afternoon on 4-8-2021.

Patient received Janssen COVID vaccine 4/9/21 around 9 am. Patient reported to emergency department around 10 pm that day (4/9)- says tongue began swelling since 7pm, he took a benadryl at 8pm with no improvement. Per ED physician, patient had "tremendous amount of tongue swelling, I am not even able to visualize the palate he has fullness to his left neck as well". Patient intubated in emergency room to protect airway. Patient admitted to ICU where he is still today (4/13). Per chart, the edema subsided on 4/12 and patient was extubated today (4/13). Patient received IV steroids from 4/9- 4/13 (discontinued this morning). Of note: patient does take lisinopril 20mg daily (start date July 2020). There is concern the angioedema was caused by this medication. Patient received tranexamic acid and Berinert in the emergency department to assist in treating this potential etiology.

Gout-like reaction (self-diagnosed) at joint of large toe of right foot. No previous history of gout. Symptoms include significant swelling and joint pain, redness and warmth at affected area. Condition persisted 3 days; condition has begun to subside (at present date, only minor swelling remains).

Patient called reporting right lower extremity swelling that started in his right ankle about 45 minutes after his vaccination. The swelling gradually went up to his calf.

10 days after the shot, my blood pressure rose to 198/112 and my pulse went to 161. and i called 911 and went to hospital. i had a severe headache for 3 days leading up to calling 911. finally on the April 1st, i went to hospital. i had a lump on my right shoulder never there before and i felt like the lump was causing my right side of my face to be numb. The hospital refused to believe me that i had some blood clot, they downplay the incident and instead gave me a brain CT SACN and brain MRA which was negative, they kept me for observation overnight and let me go noon the next day. since i have had follow ups with my regular doctor, a cardiologist and my nephrologist. I know that reaction was due to medication, either way glad i got the shot. BUT Since this time, I am retaining water, i know this because my rings do not fit anymore, rings i had for 20 years, never ever once had a hard time to put on or remove. I know my body, thats not the worse of it. i test my blood pressure every day at requesting of my kidney doctor, since the incudent of my pressure 198/112 my diastolic on average is raised 20 points. I log this daily. here are some examples from before the medication 111/67 , 109/63 , 129/64 , 105/62 120/81. now my pressure is totally different for example, 155/88 150/91 157/90 , 134/88. nothing else changed in my life, I am exactly the same as before only now i have covid shot.. yesterday i saw the 4th doctor since i am shot up and she gave me a new pill olmesartan medoxomil, only on it one day but my diastolic pressure is still much higher than before. i dont want to die, i dont want a stroke, or a heart attach, i have a lot to live for still. nobody is helping me, doctors are just throwing pills at me and not treating root cause which i feel strongly to beleive it is from covid shot.

I have severe swelling in both legs from knee to my feet, severe itching and hives, legs have blistered and burst open. I also have a Blood clot in my lower left leg. Have tried 2 kinds of antibiotics, I am on Xarelto 10 mg tab for the blood clot

April 7th cold chills lasted 20 min. April 14th a rash on both forearms and both thighs. Also had swelling of feet and hands with ankle and wrist joint soreness. April 16th started taking 4 Benadryl a day. 3 days later rash gone. Swelling and joint pain also gone.

In the beginning a very strong pain arms whole body after the stomach started the back and everything got worse for 4 days going to and from the emergency room and then still hospitalized all due to problems after the vaccine in the last exam the variant was positive for guillen barrer before discovering this he lost his sight for two hours he cannot walk he lost the strength of his limbs and his pressure shot up he has dysphacia and is still hospitalized with physical therapy

Patient developed paresthesia/sensory changes and lower back and abdominal pain. Found to have likely Guillain Barre syndrome. No motor involvement and discharged home.

2 days post vaccine patient developed lower extremity edema which over the next 3 weeks progressed to anasarca and renal failure requiring hospitalization, renal biopsy and eventual hemodialysis.

3 weeks after vaccine, developed bilateral lower extremity pain with right lower extremity swelling. Seen by PCP. No new treatment. Symptoms lasted 3 weeks.

Developed 3 small blood clots in his left lower leg on Saturday, 3/10/2021 - swelling and slight redness along with pain

Immunization was administered at 1pm, and patient started to go into anaphylaxis at 10:00pm. He swelled up, and had trouble breathing. He received epinephrine, steroids and breathing treatments in the ER.

started getting numbness in hands-fingers and lips and then numbness to feet and fell to ground with inability to walk - no strength in legs knees or arms. left facial drooping and slurring of speech and severe weakness -paralyzed

The Jansen vaccine was given on 3/18/21. Patient began to experience symptoms of bells palsy on 4/6/21. Patient noted R hemibody numbness, which began with right-sided facial numbness when brushing teeth. He was evaluated by his PCP (no exam noted) and initiated on Bells Palsy regimen of doxycycline 100mg BID, acyclovir 800mg TID, and prednisone 40mg. On 4/9, a referral to neurology was made after pt returned to clinic w/ burning deep low back pain that radiates up to his shoulder, and his b/l temples. Pt again re-presented to urgent care on 4/12 due to severe burning back pain that resolved w/ activity. At this time again only L sided facial weakness noted (documented as reduced ability to close eyelid, raise eyebrow, weakness puffing out check, and reduced smile on L), no other weakness, drift, or sensory findings. Pt was instructed to continue doxycylilne 100mg BID 10d course (4/6- 4/15), prednisone 40mg 7d course (4/6- 4/12), acyclovir 800mg TID 7d course(4/6-4/12).

Swelling in right leg following workout. Became painful as day went on. Swelling, pain continued. After three days, visited doctor.

After my vaccine, I experienced redness sensation to my arm. My left arm was swollen and raised bump with a rash. I called my doctor and he thought it was possible cellulitis. He prescribed Prednisone and an antibiotic for seven days. After the seven days my rash was gone but I still had the raised bump. I went to an urgent care facility where they performed an ultrasound of left upper extremity and no blood clot was found. I was told to apply warm compresses to my left arm once a day. To this day I still have an aching sensation to my left arm and is also sore and my left arm is still red but flat in shape.

Patient had onset of right calf pain and swelling several hours after receiving vaccine. Was seen on 5/10 and diagnosed with right lower extremity DVT.

MINI STROKE IN LEFT ARM, HAS LEFT ARM WITHOUT STRENGTH, NO MUSCLE CONTROL OF 3 LEFT HAND/LEFT SIDE FINGERS. ARM/WRIST JERK UNCONTROLLABLY. DR. VISIT 8 DAYS AFTER STROKE. LAB TESTS DONE. PRESCRIBED atorvastatin 40 MG 1 PILL AT NIGHT. URGENT CARE VISIT 5/18/21 LEFT HAND AND FINGERS UNCONTROLLABLE, HAND HAD SEVERE SWELLING. PREDNISONE PRESCRIBED

Patient presented in the office on 5/12 complaining of edema. The edema involves the left lower extremity. Onset was gradual 2 month(s) ago. Onset followed starting a new medication (around J and J vaccine). The symptoms occur intermittently. The patient describes this as unchanged. Symptoms are exacerbated by prolonged standing. Symptoms are relieved by leg elevation. Associated symptoms include calf pain (in upper thigh) and calf swelling, while associated symptoms do not include dyspnea, decreased urinary output, ascites, diarrhea, localized redness, localized warmth or fever. No chest pain

Admitted with c/o generalized weakness and significant pain to BLE s/p J and J vaccination. Neurology diagnosed pt with Guillain-Barre syndrome and is s/p IVIG and steroids'. Pt improved with IVIG and steroids'. Of note pt. states he has been to the hospital multiple times with these symptoms and told he had rhabdomyolysis 2/2 statin use therefore it was d/c on prior evaluations and he was d/c home. Further evaluation showed pt has Guillain-Barre syndrome.

apr 24, 21 woke up with swollen face on right side and numb face and fingers and toes. went to dentist 4/27 dr said massive infection, got keflex 500 2 stat, then 1bid , took for 10 days. 4/30/21 full facial paralysis, hands numb, feet numb from arches forward and burning pain in hips and legs and lower back and headache. went to er 5/3/21. they diagnosed w/ lyme disease. started on doxycycline 100mg twice daily for 10 days and asked him to finish keflex too. pt reported zero energy. Went to dr. early 5/7/21 for results from tests run at hospital. Early didn't have results and extended doxycycline to 21 day treatment. (21 day doxycycline and 10 day keflex treatment were both completed) Pt asked early to refer to Dr. pt saw dr 6/1/21. Dr diagnosed w/ guillan barre syndrome. drew labs and did mri and spinal tap. started immunoglobulin tx, plasma transfusions on 6/13/21 for 5 days. had some improvement but currentyl still has partial facial paralysis, hands are tingly but not numb, feet are still numb, pain in hips and legs is gone. after tx, DR said results are not conclusive but there are certain aspects of lyme but he does have gbs. not clear if from shot or tick. tick discovered 4/15. tick was attached.

Sudden onset of severe headache, chest pain, difficulty breathing, fatigue and nausea, ~72 hours post vaccination. After 16 hours of bad symptoms that got worse, I took my husband to the ER, as he was having difficulty breathing and moving. (He is normally very fit, active and healthy). In hospital, they ran a battery of tests and found signs of inflammation in chest and lymph nodes. They gave him fluids, and intravenous pain meds (acetaminophen, Toradol), which helped slightly. Symptoms diminished slowly over next 24 hours, and he was feeling 90% better by Saturday afternoon.

Patient received J&J COVID-19 vaccine on 3/26/21. Due to breakthrough infection, he was hospitalized from 7/9 to 7/12/21. Patient with sepsis presumed from COVID-19 infection. Blood culture negative at time of admission. Patient was weaned off O2 and did not receive steroids. Received empiric vancomycin and cefepime.

puffy hands and feet (inflammation) -- shortness of breath (1 night), Headaches (mild), lethargy (2 days)

Severe lower bottom left of the abdomen on March 26 at around 7am. Difficulty in walking. Pain around hip and middle and upper back.

Pt reported to Urgent Care with paralysis and numbness on his left side. Left-sided neck pain radiating into the top of the shoulder.

I received the Janssen vaccine on March 13. By the next weekend, I had what I thought was a spider bite on the inside of my left thigh. That "bite" got worse--bigger, hot to the touch, and then red. On the weekend of the 27-28 of March, it was red, raised and very painful even when not touched. I called my family doctor on Monday morning, the 29th of March, and I was sent to the Emergency Room, where I was diagnosed with a 7cm thrombosis in my left greater saphenous vein, which is treated the same as a DVT. I was treated first with Lovenox injections, then Eliquis 10 mg 2x daily, now 5 mg 2x daily. The blood clot is still painful to the touch.

Patient started with Left lower leg pain and redness on 4.1.21. Went to Emergency dept on 4.4.21 and diagnosis of Left leg cellulitis and prescribed Doxycycline. Patient came to PCP office on 4.6.21 with worsening left lower leg pain and swelling. Order L lower leg doppler. Diagnosis of L lower leg deep vein thrombosis of tibial vein on 4.7.21 and prescribed Apixaban 5mg 2 BID x 7 days on 4.7.21

Abrupt onset of widespread joint pain, stiffness, significantly limited range of motion within a few days of receiving his covid vaccine -- > PMR like symptoms -- > elevated CRP

The patient's past medical history included ablation, and stents, and concurrent conditions included heart issue, non-alcoholic, and non-smoker, and other pre-existing medical conditions included the patient had no known allergies and no drug abuse or illicit drug usage.The patient received covid-19 vaccine (suspension for injection, route of admin not reported batch number: 1808986 expiry: 04-JUL-2021) dose was not reported, administered on 24-MAY-2021 15:00 for prophylactic vaccination. No concomitant medications were reported. On MAY-2021, the subject experienced right hand swollen. On MAY-2021, the subject experienced unable to close hand completely. On 25-MAY-2021, the subject experienced right hand cramping and pulsating pretty good. The action taken with covid-19 vaccine was not applicable. The patient was recovering from right hand cramping and pulsating pretty good, right hand swollen, and unable to close hand completely. This report was non-serious.

I suffered the worst herpes zoster infection I have ever had. An infection resulted in a scab, below my lower lip, that was about an inch square. It was HUGE. I also know that herpes zoster outbreaks after the vaccine are a sign that the vaccine has weakened my immune system.

Patient has had intermittent weakness since covid vaccine in April. Admitted on May 11 for weakness, stroke was ruled out. Noted to have bilateral upper and lower extremity paresthesia thought secondary to diabetes. Readmitted on May 17, began to have frequent falls with bilateral lower extremity weakness, when he stood up his legs could not hold him. MRI ruled out ischemic infarction. Diagnosed with Guillain-Barre disease and ambulatory weakness. Followed up with neurologist.

Injection site swelling and moderate pain within 20 minutes of injection; within 30 minutes the left forearm, left metacarpal region (in particular, tissue surrounding base of left pinky finger) experienced swelling. Left calf (gastrocneuius) experienced mild, yet noticeable, swelling which diminished and was gone within 8 hours. Pronounced fatigue set in within 3 hours after injection and I was in bed the following 2 days. Mild, continuous ringing in the ears began on day 2. By day 4 was back 100% to a normal routine. The only remaining symptoms as of the date of this report are the mild, continuous ringing in the ears and the small, yet noticeable swelling of tissue around the base of the left pinky finger.

Slept badly 9-Jul-2021. Woke up with strong flu-like symptoms - fuzzy headed, ears hot (probably fever, but thermometer broken), total lack of energy. Did not make it out of bed beyond bathroom visits and light snacks 9-Jul-2021 or 10-Jul-2021. More energy Monday 11-Jul-2021 and was able to work a few hours. I then noticed a lump on lower left neck jaw that looks like mumps, which I had as a child, and pox all over my body that look like chicken pox, which I also had as a child. I believe mumps/chicken pox symptoms began earlier, probably before the flu-like symptoms, but they were not yet severe and I did not carefully note them. Chicken pox has gotten worse through 11-12-Jul-2021 with over 100 pox, particularly bad on knees and elbows with about 70 in those areas alone. Prior to vaccination, I had taken only 1 sick day in over 20 years. I just don't get sick beyond minor cold-like symptoms.

On March 24 I began experiencing severe pain in my right arm. The er thought it might have been a pinched nerve or something along those line. March 28, noticed a lack of muscle control on the right side of my lips while drinking from a straw. This time I was admitted as a possible stroke. Two different MRI sessions revealed no stroke. They performed a lumbar puncture and discovered that I had extremely high protein levels. After having been admitted, for the next couple of days my face slowly lost the ability to close my eyes fully and a complete lack of control of my lips (total paralysis on my face). I also began to experience bouts of stabbing pain throughout my body and weakness in my limbs. After the after th

In May patient noticed progressive painful lower extremity edema (left greater than right). On June 11, patient came to the hospital with sudden onset dyspnea, palpitations, lightheadedness and dizziness while refereeing a basketball game. Ultrasound of lower extremities found non-occlusive thrombus in the left common and profunda femoral veins/occlusive thrombus throughout the left femoral vein/popliteal vein, and tibioperoneal trunk. Patient started on heparin drip and admitted to stepdown unit for concern of submassive PE given elevated BNP, DVTs, and dyspnea, and confirmed via VQ scan. Patient was transitioned to oral anticoagulation (apixaban). Swelling of legs greatly improved, and dyspnea improved though cough persistent by the time of discharge. Of note, patient also has a history of elevated PSA (to 80s), but patient has not seen urology for follow-up for possible malignancy. Patient also found to have UTI (klebsiella) secondary to his chronic urinary retention.

3/9 patient began with rash to left side of face, sore throat, left ear pain and productive cough. Was evaluated @ the facility on 3/10 where the vaccine was administered- Johnson and Johnson. Evaluated @ facility on 3/12/21. Herpes Zoster affecting left side of scalp and face extending to eyelid. Referred to Ophthalmology for evaluation. Zpak discontinued and changed to Amoxil.

Pt received the J&J vaccine on Monday 4/12 around 10 am. Reports onset of diarrhea mon night, frequent episodes. Then Tuesday 4/13 developed dysuria, fever/chills, headache, nausea, vomiting. Presented to ED 4/14 and admitted to hospital medicine with acute kidney injury.

tingling in hands and feet, headache, tired, bilateral bells palsy. Pain in arms and legs and weakness with extreme fatigue. Went to the ER, twice and was admitted. . Diagnosed with guillain-barré syndrome. Still sick.

Woke up the following morning (1 AM) with fever, chills, and considerable discomfort. This subsided by morning. The following day I began seeing swelling, redness, itching and burning of extremities (mainly the hands, but also feet, ears, face). These symptoms seem to be triggered by the slightest exertions, but have also exhibited without physical activity. These appear to be getting more prevalent each day.

3/16 - 3/23 : fewer ( 100 -101 F), headache. used Tylenol 3/24 : fewer, headache, nausea, vomiting. used Tylenol 3/25 : fewer, headache, watery eye. stayed in ER. diagnosed sinus infection. treated with Amox/clav 875mg-125mg; claritin; Tylenol 3/26 - 3/28: fewer, serious headache, double vision. continue used butal/apap/caf 50-325-40; Amox/clav 875mg-125mg; waiting for schedule MRI. 3/29 - 3/31: fewer, serious headache, double vision. stayed in hospital. continue used butal/apap/caf 50-325-40; Amox/clav 875mg-125mg.

Over a week after I got the shot I started to notice a discolored area on my elbow. It was itchy and and reddish purple. It was a base ball size area on my left arm on my elbow. I saw the doctor who said it was a vaccine reaction. I was given a steroid shot and some antibiotics. After a week it is much better with only a slight color variation.

Progressive left arm pain and extending to patient's neck and lateral shoulders peaking on 3/21/21. Numbness, tingling, and leg muscle cramps where patient went to urgent care on 3/2/21. Started on a 6 day prednisone course (40 mg daily x 4 days, 20 mg daily x 2 days) but had pain in his lumbar spine, tailbone, and hips. Muscle reflexes +1 in upper and lower extremities. Continued muscle weakness, cramping, and tingling with generalized fatigue seen in neurology clinic on 4/16/21. Admitted to the hospital on 4/21/21 for IVIG treatment and diagnosed with Guillain-Barre Syndrome.

Patient had the Janssen vaccine as noted. He presented with significant superficial clot in the lower leg. Ultrasound read Mixed partially occlusive and occlusive superficial venous thrombosis in the right greater saphenous vein. This originates 2.2 cm from the junction with the right common femoral vein. He presented with 2 days of leg swelling without clear trigger such as travel, trauma etc.

Swollen knot on left arm, Pain that travels from my left arm across my shouders into my neck, stiff neck, Left hand and finger numbness, Confusion, Painful joints, Pain in previous broken bone sites. Fatigue. Sleeplessness.

Patient started with paresthesia and weakness mid April. He was ultimately diagnosed with Guillain-Barre syndrome via lumbar puncture on May 20th after 3 ED visits and 2 hospitalizations.

High levels of pain and stiffness in joints, tendon anchorages and muscles. Initially concentrated in hips; migrating to shoulders joints, deltoids, trapezoid, front of pelvis, buttocks.

Swelling and pain in the primary knuckles of my hands--especially the right hand. Unable to make a firm fist with my right hand--weak. Ring finger on the right hand is particularly swollen and inflexible. I don't know what has caused this to occur.

No concomitant medications were reported. On 13-MAR-2021, the subject experienced left arm swollen. On 13-MAR-2021, the subject experienced left arm hurts. Treatment medications (dates unspecified) included: paracetamol. The action taken with covid-19 vaccine ad26.cov2.s was not applicable. The patient had not recovered from left arm swollen, and left arm hurts. This report was non-serious.

Pt. states that after receiving the J&J vaccine 04/07/2021, started to experience 04/17/2021 painful leg swelling, groin area, and (previous Hernia) irritant. Primary visit recommended leg elevation and compression socks. Still continuing to swell.

Patient had autoimmune appearing reaction with rash, iridocyclitis, DVT right femoral vein, thrombocytopenia, lymphadenopathy, aphthous ulcers, pharynigitis rheaumatology consultation, anticoagulation, ophthalmology care, hematology consultation, ENT consultation

After receiving the vaccine with in minutes the patients left arm began to swell and bruise. He began to become pale and his eyes rolled back into his head and passed out. He woke up a min later and had SOB. Very pale and almost threw up. Paramedics and Firefighters were called. The eveulated the patients and gave him a liter of sodium choride. His Heart Rate was in the 30's. After several min and a liter of fluid, patient was alert. Once stable he was transported to Hospital.

Patient received vaccine on 3/13, initially had fatigue and myalgia the day after, which resolved, but also on 3/14 began having mild intermittent coughing, nonproductive, occurring every two hours or less. Patient began having some blood in his sputum since this morning at 4AM, and that occurred twice more with intervals of a few hours, both continuously improving and less blood. No other symptoms. No history of TB, no exposure to sick contacts, no recent travel. Unlikely a pulmonary embolus given normal vital signs and no risk factors. Patient was doing outdoor work in his yard for the past few days before the symptom onset and was working on mulch and compost. Although unlikely to have fungal infection currently, may be bacterial pneumonia

Patient received Janssen vaccine on 4/12 and developed left arm pain a few days later. Presented to the ED with left arm swelling 10 days after vaccination and was found to have a left upper extremity DVT.

BLOODSHOT IN EYE/EYES WERE RED; KNOT IN CENTER OF RIGHT ARCH OF FOOT; NECK HURTS/COULD NOT MOVE NECK; HEAD SWELLING AND TENDER; SLEEPY; HEADACHE; CHILLS; HEART ACHE; This spontaneous report received from a patient concerned a 64 year old African American, not Hispanic or Latino male. The patient's height, and weight were not reported. The patient's concurrent conditions included blood pressure. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: 1805020, expiry: unknown) dose was not reported, 1 total administered on 08-APR-2021 between 09.30 and 09.40 to left arm for prophylactic vaccination. Concomitant medications included acetylsalicylic acid (Aspirin) for blood pressure. On 08-APR-2021, 20 minutes after vaccination, the patient started having symptoms. The patient's heart started aching and that lasted for 15-20 minutes and then went away. On 09-APR-2021, the patient had chills that had lasted for four hours. On 10-APR-2021, the patient could not move his neck left or right and back of his head felt like it was swelling and tender; he also had a headache, and he was extremely sleepy. The patient's neck and head tenderness lasted three to four days. On 11-APR-2021, the patient eyes had blood shot and were red and reported that he might had a stroke or blood clot. On 17-APR-2021, he had a knot came up in the center of right arch on foot. On 19-APR-2021, at the time of this report, the patient's eyes were still red and had a slight headache; the patient's neck still hurts, he was sleepy, and the knot was still there on his right arch of his foot. The patient took acetylsalicylic acid (Aspirin) but he was already taking that for his blood pressure. The action taken with covid-19 vaccine ad26.cov2.s was not applicable. The patient recovered from heart ache on 08-APR-2021, head swelling and tender on 14-APR-2021, and chills on 09-APR-2021, and had not recovered from neck hurts/could not move neck, bloodshot in eye/eyes were red, headache, sleepy, and knot in center of right arch of foot. This report was serious (Other Medically Important Condition).; Sender's Comments: V0: This 64-year-old African American (non-Hispanic/Latino) hypertensive male patient reported "heart aching" 20 minutes after receiving COVID-19 VACCINE AD26.COV2.S for the prevention of symptomatic SARS-CoV-2 virus infection. The event lasted for 15-20 minutes. Other associated symptoms developed in the succeeding 3 days: chills that lasted for hours, could not move his neck and felt it was swelling and tender, headache, sleepiness, blood shot eyes. Nine days after vaccination, the patient reported he had a knot in the center of arch right foot. The patient took Aspirin. No other details was reported. The event is confounded by the underlying hypertension. The information available precludes a complete and meaningful assessment. However, considering the temporal relationship and recently evolving theories in the literature about COVID infections and vaccinations, potential vaccine contribution cannot be excluded. Additional information was requested.

4/25/2021 - He was apparently well until 2 days prior to admission when he developed spontaneous left calf pain and swelling. He denies any associated trauma, recent travel and immobility. He is not taking any new prescription, including steroids. One day prior to admission, he started experiencing left-sided pleuritic chest pain up to 9/10 in intensity and exertional shortness of breath. He denies any fevers, hemoptysis, sputum production, diaphoresis, nausea, vomiting, abdominal pain, dizziness or lightheadedness. The pain was persistent throughout the day. Today, his chest pain persisted and his shortness of breath had progressed. He also noted left arm swelling with mild discomfort. He felt that his left leg swelling and pain had improved today. Nevertheless, given persistent symptoms, he decided to come to the emergency room for further evaluation management. In the emergency room, his initial blood pressure was 218/104 with mild tachycardia of 99, normal oxygen saturation and no fevers. CBC showed a white count of 12.35, hematocrit of 50.4 and normal platelet count 198. INR was normal at 1.06. BMP showed mild hyperglycemia at 148. Troponin was 5 and BNP was 8. Imaging studies show extensive left upper extremity DVTs that involved the left subclavian vein, left lower extremity DVTs and bilateral lower lobe pulmonary emboli. 4/30/2021 Presumed unprovoked extensive DVT of the left upper extremity and left lower extremity. However, he did recently receive Johnson & Johnson Covid vaccine. Now status post catheter directed thrombolysis to the left upper extremity.

Guillain-Barre Syndrome - mild case. Tingling, numbness up entire legs more prominent in feet. Mild back pain. No weakness. Some numbness and paresthesias hands. Restlessness.

Initially I experienced nerve pains across my body, including under both of my armpits, in my groin area, in my arms including my triceps, forearms and they were very sharp pains. Then I started to get a severe headache that was pretty constant. After multiple days, my face became paralyzed. I also experienced severe fatigue. This was over 10 days. Once paralysis happened we went to the Medical Center. I was treated with IVIG for many days and oral steroids. I was in the hospital for 6 days. I also experienced my BP spiking. I had an abnormal EKG. I'm still unable to chew properly. I was told that now I have Encephalitis and GBS (Gillian Barres syndrome).

ONCOLOGY/HEMATOLOGY SUMMARY: -4/7/2021. Patient received COVID-19 vaccine Johnson & Johnson. A week later he developed an eruption on his trunk which resolved followed by extensive bruising and asymptomatic mucosal bleeding. -5/4/2021. Leukocytes 7.4, hemoglobin 14.9, platelets less than 5K, creatinine 1.02, liver function within normal range. Initiated dexamethasone 40 mg daily x4. -5/7/2021. Platelets improved to 31K, leukocytes 16.6, hemoglobin 14.5, CRP 3.6, ESR 4. Negative serology for HIV, Hep B, C, tick borne disease. Normal ANA. Normal b12, folate levels, low TSH but normal freeT4. -5/10/21. Platelets decreased to <5K. Started prednisone 1mg/kg at 80mg daily with poor response, thus tapered off in 15-20 days. -5/17/21. Bone marrow biopsy. Results pending. -5/18/21-5/20/21. IVIG 1gm/kg x 2. - 5/24/21. Improved platelet count to 133K/uL

Patient had Janssen COVID-19 vaccine on 5/8/21. Developed calf tightness, pain, and swelling 5/13/21. Sought medical care 5/20/21. Diagnosed with DVT 5/20/21

On 6/7/21 I noticed my right lower leg was swollen. On 6/9/21 I went to my PCP and she sent me to get an ultrasound. The Ultrasound showed a large DVT and I was sent to emergency room. In emergency room a Cat scan of my chest was completed and showed several pulmonary embolism. I was admitted overnight and started on Eliquis. I was told to follow up with my PCP.

15 min. after receiving vaccine in left arm, felt tingling in left hand. Two weeks later left arm went completely numb, temporary loss of fine motor control, ok after a couple days. About a week later arm went numb with an icy cold prickly feeling. Lasted about a week, with a little residual numbness still present. About May 1st, also noticed swelling of left ankle. During the month of June had pain behind knee and continual pain and stiffness in left leg. End of June left leg became noticeable swollen. Went to Urgent Care on June 30, 2021. Sent for ultrasound, found four blood clots (DVT's) in left leg (3 upper thigh and one behind left knee). Sent to ER, started on Eliquis, will follow-up with primary care next week. No history of blood clots before, and I am quite active.

I woke up with severe face swelling it continued to my lips and then my tongue . I went to hopital . I was there for around 14 hours until they could monitor swelling and ensure my throat was. not obstructed . I was referred to an allergist that did blood tests and determined i have no allergies. He prescibed an EPI PEN and steroid to use if it occurred again . Symptoms did not go away for about 5 days . Since then I have had 8-10 episodes of severe swelling and welts all over my body.TodayI woke with the left side of my face very swollen and welts on my thigh. I am taking the benadryl and steroid he prescribed. The

patient developed right lower extremity DVT diagnosed ultrasound 7/20/21. He started with acute pain right leg Saturday 7/17 and woke up on Sunday 7/18/21 with swelling right upper thigh and bruising and swelling of right foot. His swelling and pain increased and was seen urgent care where he was referred to emergency room for evaluation DVT.

Intense Lymphadenopathy, left neck below the ear, hard, warm, fever, headache, swollen, painful. Am onset impaired neck movement but no challenge to swallowing or breathing.

Patient presented to clinic 9 days after vaccination in acute distress, shivering, tachypneic and having difficulty with speech. Heart rate at 130 beats per minute, Spo2 91% on room air, temp 99.9f oral. Initial difficulty obtaining blood pressure due to shaking. Patient was treated with 500mL NS and 2L O2 via nasal cannula, which immediately improved symptoms. Due to elevated glucose and minimal austere conditions, patient was evacuated from camp to hospital. There, patient diagnosed with COVID-19 infection, bilateral basilar and right apical pneumonia, diabetes and sepsis.

Patient presented to ED on 4/14/2021 with 24 h of progressive painful/pruritic palpable purpura of distal upper and lower extremities. No evidence of additional organ involvement.

Patient right leg became very swollen from knee down and was very painful to walk. Patient went to Urgent Care and was told that he would need to have ultrasound done to see what was going on. Patient went to Imaging and was told that he had several blood clots through out his right leg. They advised him to go to emergency room evaluation. Patient went to Medical Center and it was confirmed that patient had blood clots and was put on Xarelto for treatment.

On April 16 patient came home fatigue, pale and legs hurting. The next day complained of right knee pain, Fatigue and looked pale. He woke up in the middle of the night complaining of right calf tightness and pain. On Sunday 18th he started to limp and when we looked at his calf it was double in size, warm to touch, and redness. We then went to the ER and had an ultrasound and CT scan . We were told he has multiple blood clots in the right leg and clots in both lungs. He was then transported to a hospital and given blood thinner shots in stomach. He was released from the hospital 4/20 and given blood thinners and assigned a hematologist. He?ll be on blood thinners 6-12 months, out of work a month and tested regularly to check platelets.

Lump in my right calf 7 days after the vaccine. Ultrasound indicated it was not DVT. Waiting for Radiologist report.

Patient developed ataxic gait determined to be Guillan-Barre syndrome. Was treated with course of IVIg with improvement but not resolution.

About a week after the shot, I started getting redness, tenderness, discoloration, and swelling of my right arm. The discoloration began to spread and I went to the doctor on the 21st. The doctor said there was inflammation and because it was warm to the touch, was concerned about blood clots. I was sent for an ultrasound and prescribed an anti-inflammatory medication and advised to take aspirin. Everything cleared up about a week later.

On April 7, my husband received the J&J COVID vaccination. On April 17, he started to feel ill and developed severe abdomen and back pain, which were not normal symptoms of his other health conditions. He ate very little on that date (and that was the last food he had). At 3:30 a.m. on Monday, April 19, I took him to the ER because of severe pain. They gave him multiple tests, including CT scan, labs, EKG, ultrasound, and others. He was admitted to the hospital, and multiple other tests were performed during his stay. During his stay, his sodium and other electrolyte levels dropped to dangerously low levels, he experienced major swelling, his pain persisted, his swallowing ability diminished, he didn't eat, and his heart consistently ran at a high rate. He was transferred home (with comfort care/hospice) on Tuesday, April 27, and died on Wednesday, April 28, at about 9:30 p.m.

Right leg swollen, went to ER on July 17, 2021and was diagnosed with Deep Vein Thrombosis (right leg - blood clot). Currently taking Xarelto 15 mg 2x a day for 3 weeks, then Xarelto 20 mg for 6 months.

Approximately 4-5 days after vaccination, the skin of my face, scalf and around my eyes became itchy. I think there's very minor puffiness of my upper cheeks at either side of my nose. Now, ten days later, the itching persists.

By 10:00 pm that evening I got Chills, fever, muscle aches, headache, nausea and low back pain. Stayed home from work for 1 or 2 days. Felt better in all ways except my low back got very, very painful. I asked my wife to buy me a walker to take to work because I had difficulty walking the distance from the parking lot to inside my office. Also used the walker at work. My boss said to go home, it looks like you're in pain. Saw Dr. on Monday March 15 because the low back was getting worse. He prescribed muscle relaxors and anti-inflammatory medicine. Called the next day asked for something stronger for pain because worse. Nurse responded to continue on existing meds. Went to Chiropractor on both March 16 and 18 without back pain relief. On March 19 went to urgent care, got X-ray and pain meds. MRI done 2 days later on March 21st which revealed a spinal abcess. Admitted to Hospital, surgery done to remove spinal abcess on March 22. Bacteremia, laminectomies in L3,4,5 S1 to relive spinal cord pressure. Puss had the filled spinal epidural cavity, cultured out as Strep bacteria. Remained hospitalized until April 2nd. Sent home for 1 night then returned to hospital via ambulence the next day April 3rd with 103 degree fever. Discharged April 8th. Readmitted to hospital for 3rd time on April 17 where I remain today with Kidney and liver function problems likely reactions to meds. Continue to have excruciating low back back which is why continues to be hospitalized at the time of this report.

The second week after I got the vaccine I started to have a pressure headache on the side of my head and down my neck. My leg was painful by my knee and it lasted a week. I had the headache and the pain at the back of my neck for one week. The pain medication did not help with the pain. The headache was on both sides but much worse on the right. I had a bump on the back of my neck on the right that was painful. I had a rash on my arm around the site of the injection about an inch around which is still visible after three weeks. The rash was a little itchy.

Swelling of both hands and arms. Hives presenting on both arms and wide spread itching and redness. Referred to physician. Outcome unknown at this time.

That day, light headed ness and heart palpitations. Heart palpitations for short time only but felt periodically over the next few days. June 11, lower left led swelled and painful and shortness of breath. June 14, went to ER. They found multiple blood clots in upper thigh, behind knee and lower left leg. They also found approximately 20 clots in the lungs and administered Heparin drip and I was impatient. They put me on Eliquis and released me the next day. Now convalescing at my daughter?s house. Diagnosis. Acute Deep Vein Thromboses and Pulmonary Embolism bilateral.

Laboratory data included: Blood pressure (NR: not provided) Normal. The action taken with covid-19 vaccine ad26.cov2.s was not applicable. The patient had not recovered from swelling in both ankles, and swelling in feet. This report was non-serious.

Mild fever, chills, fatigue and brain fogginess I first 36 hours. At 48 hours developed temporary hives, swelling, ichiness in right abdominal area and genitals. This took two additional days to resolve with no subsequent recurrence.

Nauseous, vomiting, headache, light headed, disorientation, vision changes, jaw pain, lymph node inflammation, lethargy, mental confusion, sense of hopelessness, 15 days taper Prednisone, ibuprofen, acetaminophen, still currently experiencing symptoms today

Patient developed acute onset bilateral anterior thigh pain that radiated to the lower back and buttocks, as well as in the posterior hamstrings. The pain was 10/10 and started suddenly over 1/2 days, described as sharp and stabbing in nature. He was able to ambulate but had extreme pain with hip flexion. Associated with this had unintentional weight loss of 10 lbs over 3 weeks, loss of appetite, drenching night sweats for 4 days prior to admission. He presented to the ED on 4/16 with negative work up and was discharged. He re-presented on 4/24 with the same symptoms and was admitted until 5/3. His pain was managed with oxycodone and extensive work up was pursued given concern for systemic inflammation with leukocytosis, elevated CRP, ESR, but normal CK. Eventually pursued thigh muscle biopsy which showed ". At last point of contact at 5/7, he described having two more episodes of bilateral thigh pain after discharge that lasted 30 min in the mornings, but had not had any symptoms the day prior to the call.

Received shot at 10:00am on 5/5/2021. On 5/6/2021 Patient began to feel ill with fever, chills and body aches. Spent most of that day in bed. On 5/7/2021 presents to Clinic, fevers, chills and body aches had improved but noticed "rash" - right lower extremity was red, hot to touch, swollen, pain with "tightness" and "burning" sensation at site. Denies itching. Right Lower Extremity well demarcated area of erythema, hot to touch and remarkably TTP, particularly at anterior surface of leg. Erythema extends on anterior surface of leg from ankle up to 8cm inferior to the knee cap ;the erythema on the posterior surface of the leg extends from ankle up to about half way up the calf. Erythema is confluent. Anteromedial surface of the skin has diffuse petechiae. Negative Homan's sign and no calf tenderness, 2+ pitting edema from ankle to midway up the shin.

Had a heart transplant 5 years ago. After vaccination, my calf muscles ached and I had to walk on the balls of my feet. I didn?t feel well and was achy, but all of that resolved after about 4 or 5 days. About 25 days after the vaccine, I woke up and my legs were hurting, swollen and lumpy. I went into the house and elevated my feet. I was sore from my calf to my ankles. I called the doctor and he told me to go the ER. After a 3 hour wait, one of the patients waiting almost threw up me, so I told them I was going home. Later, I went in to see the doctor about what was going on. After rubbing my calf vigorously, some of the lumps started to go away. The only thing my doctors came up with was that I was retaining water. I felt like I had gained some weight. I couldn?t stand long before my legs swelled. They prescribed a strong diuretic called Torsemide and I lost about 10 lbs. I currently wear compression socks every day, which helps with the swelling and my ankles hurt all of the time. I only take the meds as needed because it?s hard on my kidneys, which are already compromised due to my transplant meds.

Increased fatigue, lightheadedness, nausea, intermittent left arm pain and underarm pain and swelling. Left sided chest discomfort - pain worse with breathing and coughing non productive cough

Approx. 1 week after vaccine administration, developed severe pain in my right shoulder area. The right arm received the vaccine. No pain prior. All felt well. Went to see primary care doctor who said I have sub-deltoid bursitis possibly from vaccine shot. Gave me an injection of Triamcinolone (steroid) in my right shoulder. This was done on 6/8/21. Today is 6/15/21 and still have pain but not as severe as initially. Still hurts when moving my shoulder or right arm. I can hear snapping noises in shoulder area when I move my arm around. Difficult to sleep due to the pain. Have been doing range of motion exercises. I Notified public health about bad rxn. but no one has yet returned my call

Developed myalgias and joint pains approximately 48 hours after the injection, then improved. Then about a week after the injection, developed severe myalgias and joint pains. Treated with Medrol dose pack (6 days) with improvement. 3 days later developed pleuritic chest pain as well as shortness of breath. Presented 48 hours after this started for medical care and was diagnosed with acute pericarditis. Started on ibuprofen 600 mg TID and colchicine 0.6 mg BID with significant improvement in discomfort. Discharged from the hospital after 48 hours of observation. Did develop paroxysmal SVT which was felt to be 2/2 to the acute pericarditis. Started on metoprolol tartrate 50 mg BID to help with rate control and sent home with zio patch for additional cardiac monitoring.

april 20th his foot started to hurt, April 21st his legs felt slightly weak, April23 & 24th felt sick had a hard time getting out of bed and dressing. April25th felt numb mid body and legs really weak had a hard time standing took him to the ER had a MRI on Back and Bloodwork done. April 26th saw his doctor ordered another MRI for head and neck. April 27th had MRI for head and neck. April 28th rushed to hospital symptoms got worse and was admitted also had a CAT scan and an MRI again. April 29th another MRI was diagnosed with GILLIAN-BARRE syndrome. Had six doses of IGG transfusions May 6th moved to rehab. was in Therapy till June 10th and Back to the hospital for 5 more treatments because his legs quit working again. WAs in the hospital for a week and then insurance denied therapy so had to find skilled nursing, found one moved him . He was at his new place maybe 8 hours then rushed back to the Hospital because a fever and may had an infection had medician to help get rid of that lost his facility he came from and then took a week to find another place he was there 3 weeks and had a EMG and the nerves in is legs are Damaged in and out so he will take a long time to heal and walk again and he is also very depressed because the whole situation.

Woke up with swelling in both hands. Right more pronounced. Slight nerve tingling in left hand and wrist. Pronounced tingling in right hand and wrist. Over the next two days the left stayed the same while the right continued to worsen to include pain and tingling from the elbow to the hand particularly in the three central fingers. Hand difficulty holding item in my right hand. I then decided to seek medical attention the next day. The next morning swelling and pain were lessened so I did not contact medical attention. Each day since symptoms have improved except for discomfort in my right middle finger which is noticeable upon touch. Most noticeable when using eating utensils. The right middle finger has remained the same for days.

On March 1st, I had a sonogram of the veins in both of my legs and nothing was found, on March 5th I received the COVID-19 vaccine after standing in line for 2&1/2 hours. By March 8th the swelling and pain in my left leg was so bad I made an appointment with Dr. for March 11th. He ordered another sonogram and found 2 DVTs in my left leg, and prescribed anti-coagulants whyich I am still on. At the time the Doctor said that this was most likly a result of having stood in line for 2&1/2 hours.

Abdominal Pain Narrative: Patient is a 70 year old male who received Janssen COVID-19 Vaccine (Lot# 1802070) on 3/9/21. Vaccine was administered with no complications and patient was advised to remain in facility for 15 minutes following administration. On 3/15/21 patient contacted hospital reporting that he has had diarrhea every day since his vaccination. Reported bowels on 3/15 turning pink and noted blood in stool with most recent BM. Patient also reports occasionally abdominal pain (4/10). States no improvement after taking Imodium. Patient was recommended to visit hospital for evaluation. Patient was admitted to hospital from 3/15 for observation with dx of leukocytosis, diarrhea and orthostatic hypotension. No complications during stay and diarrhea resolved resulting in discharge on 3/17.

Right lower leg sudden onset severe swelling, pain, redness. Went to urgent care doctor who then performed assessment, blood work and ultrasound. Placed on Xarelto 15mg twice daily for 21 days.

Patient presents to ED with left hand swelling, fever, and left arm pain on 3/10/21 after receiving his COVID 19 vaccine at Clinic on 3/4/21. I'm unable to determine the route of administration from the immunization documentation. Patient reports that the swelling significantly increased over the past day with increased swelling in his left hand and pain from his left clavicle to his left hand. Patient is febrile upon presentation with a documented temperature of 38.7 and sinus tachycardia. EKG is otherwise unremarkable. Patient has been diagnosed with left arm cellulitis and is being admitted to our hospital. He has been started on vancomycin 800 mg and Zosyn 3.375 g IV.

Patient called regarding possible reaction to Janseen COVID vaccine on Friday. Patient stated between 2:00 pm and 2:30 pm on Friday he started experiencing joint pain in his left wrist that felt like he "sprained" his wrist. Patient denies any injury. Patient also noted some slight swelling in this area. Patient took Aleve and stated the symptoms went away mid-morning on Saturday. He denies any other symptoms.

Patient developed progressive weakness and was diagnosed with Guillain-Barre Syndrome based on physical exam findings (ascending muscular weakness), lumbar puncture, and MRI findings. Disease course complicated by acute hypoxic respiratory failure which required endotracheal intubation and mechanical ventilation. He was treated with intravenous immunoglobulin x4 days.

Pt developed edema and pain to LLE from knee down to ankle on 4/9/21. Describes skin tightness and redness.. No fever, but with chills. Given crutches to avoid weightbearing on left leg. 72-year-old male history multiple medical problems including coronary artery disease with stents Barrett's esophagus asthma enlarged prostate hypertension presents from the infusion center secondary to being found to have hypotension after receiving his immunoglobulin infusion today for COVID. Patient states he has been feeling ill for the past few days due to COVID he has been eating and drinking much. Patient states that he felt no different during his infusion or after but the blood pressure being low specially felt lightheaded when he stood up he was sent down to the ER for further evaluation. Patient denies any chest pain associated with this no palpitations he is awake alert and not diaphoretic in no significant distress on my evaluation. Patient denies any recent blood in stools. Note is the patient did receive the COVID vaccine Johnson and Johnson approximately 10 days ago. He was diagnosed with COVID infection 4 days after that.

Shortly after vaccine, upper respiratory issue started. Fatigue, falling, soiling bed sheets and pants regularly. Then went to hospital on june 7th with abdominal pain. Low bloid pressure and hight white blood count. Had blood clot. Urinary Infection Possible sepsis. Still in hospital.

Patient presented to the ED on 6/7/21 with abdominal pain that had started earlier that day. CT abdomen, CXR, and urinalysis were done and all came back negative. Patient denied any respiratory symptoms. On physical exam, he was found to have severe swelling, erythema, and ecchymosis on his left leg extending into the upper thigh and knee. A lower extremity doppler was done and found extensive occlusive thrombosis within the left lower extremity beginning in the external iliac vein and extending into the common femoral, femoral, profunda, popliteal and visualized deep calf veins. No evidence of PE or DVT in the RLE was found. Vascular Surgery was consulted, who did not recommend any procedures, but patient was started on a heparin drip. Hematology later saw the patient and changed the heparin drip to rivaroxaban on 6/9/21. Infectious Diseases was also consulted for possible sepsis.

feet got sore, became painful icy hot eased pain but it returned pain worsened felt like needle stabbing feet swollen feet

swelling began in feet and moved up legs to abdomen; 25 lb weight gain in two weeks between routine doctor exam and emergency visit leading to hospital admission for 6 days where an atrial flutter was detected that was not present in May 19th exam. Diuretics in the week since admission have pulled off over 35 lbs and vastly reduced swelling but heart damage remains.

Patient presented to the hospital with left leg pain and swelling in the groin region as well as fever. Xrays were performed and blood clotting was diagnosed. Patient presented to emergency room 21 days (03/26/2021) following Janssen COVID vaccine with one day history of increased left leg pain and swelling. At that time patient was diagnosed with DVT of left lower extremity. He was started on Xarelto for anticoagulation and has followed up in clinic multiple times.

6 days after injection woke with soreness in all joints, stiffness, fatigue. Now on day 10, the S/S are getting worst. Fingers are swollen,. I can't grasp or pinch things. I must use both hands to hold a glass or cup. Very weak, sleeping 16-18 hours a day. At times I get very chilled and start to shiver. Never have had a fever, loss of taste, or smell. I tested NEG for the virus on day 3 (10 days after receiving the vaccine). Soreness is turned into pain. Feels like full-blown RA and if I was black I would be thinking sickle-cell because of the general pain.

On 4/11 i got vaccinted vaccine Johnson & Johnson on Left Arm, on 4/19 I feel my left leg muscle pain from my left sholder to my feetand feel very tied weak body sometime my leg cramp and spasm, until May 18 went to see doctor, did ultra sound and xray on my left leg to to check for blood clut, everything negative, only at my knee have mild osteoarthrosis on 5/26 my left foot swalow and on June 9my right foot start pain cannot step on the floor, then start take 5 advil next day morning right foot pain it no more until now i still have a my muscle pain have to take Advil for pain killer

Pt was in his usual state of health. He received the J and J Covid 19 vaccine on 4/29/2021. By Mid May pt c/o sudden decline c/o sudden abdominal pain and poor appetite and lost 9 lbs by the end of may. In June the pain and poor appetite worsened and he saw his primary care provider who recommended an abdominal u/s and then a ct scan which showed liver masses/lung metastasis and lymphadenopathy . She scheduled a PET Scan and biopsy but the cancer progressed so rapidly a full work up including a biopsy and PET scan could not be done and he expired the afternoon of 7/16/21. Comparing the 2 CT scans he had on 6/30/21 and 7/13/21 show that the cancer rapidly progressed from 6.5 x 8.4cm lesion in the right lobe of the liver with no adenopathy seen and pulmonary nodules suggestive of metastatic disease to in 13 days the lesion encompassing the entire R lobe of the liver and then measuring 14 cm with no rapid progression of diffuse adenopathy and possible lymphatic obstruction. He was not a smoker (quit over 30 yrs ago per family and only smoked while in the military) and did not drink alcohol. His family denies any history of toxin exposure and He worked as a prison guard and then as a teacher. He was active prior and actively golfed, worked in the garden and was active in the gun club. He had no hx of hepatitis C. He had a past history of Melanoma s/p local incision 2008 with no history of recurrence. He was compliant with annual dermatology follow ups with no signs or symptoms of recurrence.

Patient developed rash on 3/8/21 two days following covid 19 vaccination. Red, patches, pruritic encompassing chest, back, legs, arms, and abdomen (sparing face, palms, and soles). Dermatology stated on prednisone 20mg daily to help. Due to progression, patient presented to medical center on 3/19/21. Rash appeared to have some pustules and desquamation. Also, had some swelling and pain to the areas. Dermatology biopsies on 3/20/21 which showed findings consistent with drug eruption. Rash began to improve on 3/23/21 and patient was discharged home with continued Triamcinolone cream for presumed acute generalized exanthematous pustulosis.

Approx 10 minutes after injection, pt reported his lower checks were starting to itch and he felt a little swelling in his check bones. Pt also stated he wasn't having any trouble breaking, skin tone look normal, pt was able to communicate with no problems. He stated the symptoms he was experiencing were something he was used to. It happens to him when he eats chicken and certain kinds of foods (especially seafood/shellfish). I offered him benadryl and an ice pack while he was here. He refused the ice pack, but did agree to low dose of benadryl. He took one Allergy Relief 25mg softgel and then continued to set for a little over 30 minutes total before he said he was fine and left with his wife. I contacted him around 11:50am, he was doing fine and enjoying a good lunch.

Patient sent to local hospital for venous ultrasound of left leg which showed thrombus to left distal superficial femoral vein, popliteal vein, and peroneal vein. Patient was then sent over the emergency department where he was evaluated and started on Eliquis.

Patient received covid-19 vaccine in left arm on March 9th. He alerted health care provider of left arm swelling on April 19th that started 4-5 days prior. There was no redness or warmth, reported as non pitting left arm swelling. Venous duplex revealed Left upper extremity DVT predominately effecting subclavian and axillary vein.

Approximately two weeks after vaccination I noticed his left foot and leg swelling. Patient advised me that he needed to go to the hospital because he was not feeling well and his oxygen level was low. We called 911 and was taken to the hospital. After at the hospital I advised a male nurse that I was concerned about his leg being swollen cause he had been given the Jansen Covid vaccine and I was concerned about blood clots. Everyday I would tell someone in the hospital (never saw a doctor I could talk to) about his swollen leg but did not really get anyone interested. Before he was moved to a Veteran?s State Home, I overheard someone saying they were going to give him his anticoagulant. I?m filing this report in case the swollen leg was a result of blood clots due to the vaccine.

Patient seen in clinic on 04/07/21 reporting 2 weeks of right leg swelling and calf pain. Vascular ultrasound on 04/07/21 showed blood clots in legs. Lovenox injections started 04/07/21. Warfarin started 04/08/21.

Deep Vein Thrombosis right leg. Five days after being vaccinated I began to notice swelling in my right leg from my knee through my ankle-foot area. There was an associated increase in pain in the knee. I tried to shake it off hoping that the swelling and pain would go away, but I finally relented to the concerns of my family and sought care on the 6th of April at the emergency room because my doctor refused an in-person appointment. The emergency room physician prescribed Eliquis which I have been taking. The swelling seems to have leveled off, but my right leg still appears to be twice the size of my left leg
