No convulsions , incontinence, or other seizure activity noted. EMS notified and arrived within a minute and evaluated patient. No medications or additional treatment given. Patient ambulated to EMS stretcher with one person assistance. Awake and talking. Patient transported via ambulance at 1437.

At first once I received the vaccine I felt completely fine. Later that evening I started to lose my breath and had a fever. By 9:00pm I was unable to move the left side of my body, had sharp pains throughout my body, and involuntarily urinated and vomited. This continues until 12:00pm Friday, the vomiting and urinating involuntarily stopped but the high fever, body pains, and numbness continued until Sunday evening. Now I still am experiencing random sharp pains and numbness on the left side of my body to the point where I cannot move it at all. I am throwing up occasionally (which has never happened before), and I have had a headache and insomnia since I got the vaccine. Pains behind my eye, loss of appetite and weight loss have occurred since.

I had a severe cold, with a harsh cough for over a week, extreme shortness of breath, and exhaustion. Slight fevers. I tried mucinex, but had to go to the hospital for an inability to void, with resulted in a catheter. We couldn't figure out what was wrong, so I stopped the mucinex. Then, I took doxycycline, and that seemed to help significantly. After four or five days on the doxycycline, the symptoms were almost gone.

I began feeling weak and fluey around 8:30 or 9:00 pm. I decided to go to sleep around 10, noticed my fingers had turned yellow and lost all circulation. I went downstairs to ask my dad what to do, and immediately felt like I was going to faint. Sat down on a chair and put my head between my legs, but passed out as I was doing so, and fell on the floor. According to onlookers (brother and sister, who is an EMT) I was convulsing and stopped breathing for about thirty seconds, and my eyes were glassy and rolling. I woke up to my sister telling me to breathe. There was blood on the floor (I had hit my face on the way down) and I was trembling uncontrollably and had wet myself. I was awake and aware of my surroundings, and remembered everything up to and including sitting sitting on the chair before I passed out. My brother had called 911 while I was out and an ambulance came within five minutes. The paramedics put me in the ambulance and took me to the hospital (they would have taken me to local except there was risk that it was a seizure, which I guess local isn't equipped to handle). They measured my vital signs, made IV ports in my arms and drew some blood. My blood pressure was super low and my heart rate was super high, and I had a temperature of 102. When I got to the hospital they put two liters of saline drip into me to rehydrate me, did a CAT scan, and some bloodwork. They discharged me around 3:30 AM on Monday morning, told me to stay hydrated and get a lot of rest. I slept all day yesterday (Monday) and was feverish and weak. By today (Tuesday) I think I have mostly recovered.

Midnight - I got a fever - my face was very, very hot. Fingers and toes were very, very cold. Unable to regulate body temperature. So fever and chills. Those lasted for a good four hours before it let up and then it happened again the next day and the day after that. I had about four days straight where I was not well and I don't remember a lot. On the fourth day, I was okay until the evening and I had really painful diarrhea. I couldn't keep anything in my body and I was in the restroom for about four hours. I was scared as to what was going on. I ended up calling telemedicine. The nurse I talked to over the phone and she said wait 7 days and that I should be okay. I was still slow and lethargic for the following week after but I did feel significantly better on the 8th day after the vaccine. In February, (before the vaccine) I had an UTI that I didn't go to doctor for. I drank a bunch of cranberry juice and I was fine afterwards. I do have an IUD- and on July 12th, I noticed the presence of a UTI and I had painful and really constant urinating. I was unable to hold on to anything at all and I went to Urgent care of the 14th. And they told me that I had deteriorated my bladder walls until I had come to a point where I couldn't hold on to anything. I had collapsed for lower back pain on the 16th of July and made an appt to go to doctor. I ended up speaking to someone on the 17th, and I got the medication that day for a kidney infection due to the lower back pain that I was having. And they said that the UTI had gotten so bad it had gone to the left kidney. I was first prescribed (on the 14th) - Cephalexin and had me take something called Uristat - an OTC cranberry aid and then on the 17th - they told me to stop taking the Uristat so it wouldn't be stressful on the kidneys and they gave me a different antibiotic - Ciprofloxacin and Extra Strength Tylenol. That helped with my symptoms. I had to go back in to the doctor today because for the past three days I have been having the same kidney pain again. I will get the lab results back sometimes this week. I either have another UTI or I didn't get rid of the kidney infection yet completely.

From about 4:00 p.m. until about 10:00 p.m., Patient had a headache, extreme chills, and a fever. From about 11:00 p.m. until 5:00 a.m., with varying intensity where she was able to sleep for an hour or so and then get woken up by the symptoms, she had at least a 103.5 degree fever, intense chills that caused her body to aggressively shake, excruciating pain throughout every part of her body that caused her to cry out in pain (she said it hurt more than anything she has ever felt), an intense migraine that was so bad it felt like her head was splitting open and her eyes torn out, shortness of breath and hyperventilating that caused her to repeatedly black out and weave in and out of consciousness, and extreme weakness to the point she could not move at all. She also said her muscles were aching and cramping and that her heart was pounding so hard it felt like it was hitting her rib cage. She wanted to call 911 because she didn't know how much longer she could hold on to consciousness, but was too weak to grab the phone, and her calls for help were weak and indecipherable. (Also, only her sister was home at the time, and she went to bed with earplugs in and couldn't hear patient.) From 5:00 a.m. to 7:00 a.m. many symptoms subsided, but there was still an agonizing pain in her kidneys (or adrenal glands?) that pulsed through her whole body. At 7:00 a.m. she woke up, and all the symptoms were gone, except for the area around her heart feeling strained. Two days later she called the nurse hotline and took ibuprofen as directed. The area around her heart gradually improved and seemed fine about two weeks later. Patient thinks she may have had Covid over a year ago because she remembers losing smell and having some chest pain that, for a couple of months, prevented her from exercising as she usually did. However, she never got tested because her symptoms were very mild. For her, getting Covid was a much better and less life-threatening experience than getting the Covid vaccine.

I started experiencing chills, and a fever even with Tylenol . About an hour later I felt really nauseous and then started throwing up. I threw up 4 times and then continued to dry heave for another hour. My temperature hit 102. I then started experiencing extreme pain in my right kidney area. I ended up trying to take Benadryl and then threw that right up. I then ended up going to the ER and receiving Zofran, Fluids, Tylenol, Torodol and Morphine. I got discharged with "an adverse reaction to the covid-19 vaccine and unknown pain" When I was discharged I had a fever of 100 and on the drive home I was in intense pain. It subsided but I am still not feeling well, I feel very groggy and light headed in addition to the pain.

Vaccine received 4/7/2020. On Wednesday 4/14/2020, pt went to the gym and did high intensity interval training involving body weight exercises, specifically an exercise called "v-ups". Exercise was not more strenuous than usual, and pt was well hydrated. Pt reports that she exercises often and this was not a new exercise. Soon after workout, pt began having muscular pain in the hip, radiating into her inner thighs bilaterally with associated tingling. On 4/15, pain worsened and pt had difficulty ambulating. Denies any edema, erythema in her legs or unilateral leg swelling. Presented to the ED on 4/16 and found to have elevated CK of 127,768 with UA significant for large blood and protein, without RBC, indicative of rhabdomyolysis. Pt did not have any known risk factors that may precipitate rhabdomyolysis. On admission, fluids were initiated with downtrending CK, LFTs, and renal function remained stable. er D&C was done at on 4/2 and she reports there were no complications during or after the procedure. Her BHCG was elevated at the time and she tried an oral abortion pill but still had some fetal remnants so went for a D&C. She had the J&J vaccine shortly after on 4/7 and she reports no complications around and shortly after getting the vaccine. Denies vaginal bleeding or odor. Denies fevers, chills, abd pain, diarrhea, constipation, dysuria, chest pain, palpitations, shortness of breath, nausea/vomiting.

On 11-JUL-2021, the subject experienced nausea. On 11-JUL-2021, the subject experienced tiredness. The action taken with covid-19 vaccine ad26.cov2.s was not applicable. Sunday night, my heart started pounding really hard. I went to ER - Medical Center. They did an EKG and took my vitals. I had a fever (slight) and chills and was nauseous. Put ONDANSETRON -4 mg. The nausea didn't go away and I couldn't eat or drink without vomiting until today. I couldn't eat or drink or was very tired and groggy and miserable. Today I had another trip to the ER Medical, I got on an antibiotic and anti-nausea medication (GETHALEXIN - 500 mg for the GI) (METOCLOPRAMIGE HCL - 5 mg- for nausea) from the hospital and I had a shot that went into my immune system. They also found a UTI today.

Pt received janssen Covid 19 vaccine 518pm. Pt went to the waiting area chairs and within 2 minutes she was losing consciousness. Myself and another customer were able to lower patient to the floor (did not hit head). She lost all muscle tone, consciousness and bladder control (urinating on herself/floor). Sternal rub was able to get the patient to regain consciousness. Took blood pressure at 522pm and was 152/95 with pulse of 95. Arm was hot to touch and patient was sweaty. Placed a cold pack on injection site and fanned pt until EMS arrived for evaluation. EMS loaded pt on a stretcher and took to ambulance to evaluate heart rhythm.
