rhinitis and shortness of breath with cough around 18 hours post vaccination. Now at 48 hours post vaccination doing much better with mild cough.

Within 5 minutes of receiving injection, while he was sitting in the 15 minute waiting area, patient had difficulty breathing. He started coughing. RPH injected him with dose of Epi Pen 0.3 mg & patient threw up after. We immediately called 911 and they came within minutes. Medics checked his vitals and all were within normal range.

Received first dose of Moderna vaccine today around 2:30 pm. Reports 2 minutes later, felt left side of throat "swell" and was uncomfortable. Also felt mild chest discomfort. Symptoms resolved after about 3 minutes, so he finished his 15 minute observation time, and left the clinic. He returned approximately 15 mins later, and informed the ER MD here in the clinic that his throat swelling and tightness had returned and that he now had and some difficulty breathing. He was answering questions appropriately, appeared in no distress. At 1510 VS taken: BP 122/74, O2 sat 100%, P 73, RR 18. Skin color pink/normal. ER MD advised patient be taken to ER and he called report there. Patient was taken via WC right away to ER with MD, and RN and myself. Anaphylaxis kit brought. But not used. He was stable during transport. In ER he c/o more throat tightness, and it appeared that the left side of his neck was mildly swollen. He then c/o bilateral pedal numbness and also some difficulty breathing. Care was assumed by RN and report was given to her. He was able to call his parents prior and remained stable at the time we departed the ER. NP

Pt received Moderna at 7:33 PM. Started complaining of dizziness around 7:40 PM. Benadryl 25 mg liqd po given. 8:05 PM complained of difficulty of breathing gave Epi 0.3 mg IM on R thigh, 911 called and arrived at 8:20 PM. VS stable. Pt transported to hosp.

One hour after injection, felt his eyes swell and a tightening feeling in his throat. He went to the Emergency Room and was treated and released. They gave him oral Benadryl pills and observed him for approximately 2 hours. He stated that he felt better.

15 minutes after Moderna injection, patient began to experience "throat tightness" and headache. Slight rash noted on neck area. Nursing staff responded and administered EpiPen 0.3mg IM right vastus lateralis and Benadryl 50mg IM Left deltoid once. Ambulance was called to scene and patients symptoms had begun to resolve. Patient remained alert and oriented and did not require additional support. Blood pressure was 122/90 prior to Epinephrine. Per ambulance staff, patient was transported via parent to the ER for further observation.

After getting vaccinated. Patient felt dizzy and shortness of breath, parking team sat him down in wheelchair. Medics responded, mouth feels dry. Feels a little light headed. HR 160, BP 130/85 at 9:30am. HR 151, BP 177/94 at 9:35am. Facial flushing. Loss of color in face. Transport to hospital.

Reports onset of throat swelling and difficulty swallowing which started app. 30 minutes after vaccine administered. Not reported to clinic until following day (4/8).

"At 1400 patient in observation, RN vaccinator, noticed young man's head drooped over. RN went over to the patients and tapped his arm. Did not respond to verbal or physical touch. RN called for help. After a few seconds patient responded and opened his eyes. Patient transferred via wheelchair to private area. BO 76/64, diaphoretic. RR appeared normal. Patient stated he did not eat all day. patient stated his throat hurt and felt tight and he had chest pain. Patient agreed to be transported via on-sight ambulance to Medical Center for assessment with brother."

History of Present Illness: Patient is a 18 y.o. male with no pertinent past medical history who presents to the ED with complaints of chest pain. The patient notes that he started noticing chest pain this AM after he woke up that is associated with nausea. The pain is described to be in the center of his chest and is persistent. EMS was called who administered 2 SL NTG and 1 dose of ASA 325 mg. He notes that he received his 2nd Moderna vaccine on Thursday April 15th. He is currently a senior in High school. He had no adverse reactions to his 1st dose of the Moderna vaccine but after the 2nd dose he notes that he was having fever, malaise, body aches, and decreased appetite. He also tells me that he has been so fatigued that he has not really gotten out of bed in the last 48 hours. In regards to his oral intake he had (1) 16.9 oz bottle of water yesterday and nothing else but did have a glass of orange juice and some cereal this morning. During my evaluation with the patient he notes that he is still having chest pain that he reports to be retrosternal any 5/10 in severity. He denies any radiation into the left arm or jaw. He feels the pain is worse with movement and better when he is resting. He has had associated nausea and shortness of breath. He has been having low-grade fevers at home but denies any coughing, abdominal pain, diarrhea, dysuria, burning or itching with urination, congestion, rhinorrhea. He denies any tobacco use, drug use, alcohol use. In the emergency department initial workup is notable for potassium 3.6, magnesium 1.7, phosphorus less than 1, troponin 7206 with repeat at 10,212. CPK noted to be 380. Initial EKG was nonischemic. COVID swab was noted to be negative. After I evaluated the patient around 1520 an RRT was called due to hypotension and bradycardia. His blood pressure had dropped to 80/40 and heart rate was in the low 50s and he was symptomatic with diaphoresis and feeling as if he was going to pass out. 1L fluid was given running wide open. Repeat EKG did show some diffuse ST segment elevations concerning for pericarditis. Cardiology and ICU team were notified and evaluated the patient. Response is likely that the be vasovagal from acute pain. Acute mental status change ? Acute illness/medical condition ? chronic dementia/mental challenges ? poor insight ? other ____

He was admitted to Cardiovascular Medicine on 4/11/21 with acute idiopathic myocarditis, the etiology was unclear but considered due to recent COVID vaccination vs viral infection. He presented with elevated troponin and initially elevated ESR and CRP. CTA of coronary arteries was on done on 4/12 and showed normal coronary arteries. TTE was done on 4/13 and showed a normal LVEF of 65% and normal LV size and wall thickness. His RV size was mildly enlarged and has mildly elevated right atrial pressure. There was no significant valvular disease. Cardiac MRI was done prior to discharge and he will follow up with cardiology for the final results. He was having some intermittent chest pain and SOB and was recommended to use OTC ibuprofen 600 mg BID per cardiology. Reports that his chest pain and shortness of breath is slowly resolving. His symptoms are mild and infrequent.

When client returned for his 2nd Moderna dose, on 4/26/2021, he reported to the nurse (vaccinator) that he experienced shortness of breath and difficulty with breathing, and headache after he received his first dose of Moderna vaccine on 3/29/21. The client affirmed that these symptoms started within a 4-hour period after receiving the vaccine, but after he had left the vaccination center. He said that the symptoms lasted about 2 hours and then resolved. He reported that he did not seek medical care. Vaccination with the 2nd dose of Moderna was deferred and client was directed to see one of the allergists here for evaluation prior to getting any additional COVID19 vaccinations- he said that he could do this and he was given the names of the allergists and instructed to call them or his family doctor (he has one).

The patient presented with chest pain and dyspnea on exertion which started the day after receiving the second dose of the Moderna Covid vaccine. Pt was found to have myocarditis. He was admitted to the hospital and treated with NSAIDS and Steroids. He improved and was discharged after 2 days.

Pt developed chest pain , shortness of breath 2 days after initial COVID-19 vaccination he was admitted to Hospital with myopericarditis

After 2nd vaccine dose developed myalgias, fever, chills, chest pain and shortness of breath hours afterwards which persisted and continued to worsen. Initial work up so far in the hospital appears to be myocarditis and is he is being admitted to telemetry.

ABOUT AN HOUR AFTER THE SHOT, TIRED AND WEAK. ARM SORE. NEXT DAY WEAK, DIZZY, COULDN'T GET FULL BREATH....NEEDED BREATHING TREATMENTS ALL DAY, HARD TO LIFT ARM, TRIED TO WALK BUT FAINTED AND CRASHED INTO A WALL.

Dose 1: 4/29/2021 Dose 2: 5/27/2021 Symptoms onset began on 4/28/2021 and included fevers, cough, weakness, heartache, diarrhea, headache, dizziness. Symptoms temporarily improved on 5/29/2021. On 5/30/2021 at approximately 1:00 AM patient experienced constant, pleuritic chest pain/pressure and shortness of breath which prompted him to go to the emergency department.

On 6/1/2021 patient presented to the Medical Center with C/O chest pain, diaphoresis, and nightmares during his sleep. Physical exam revealed mild tachypnea.

Patient developed urticarial rash 1 week after receiving 1st dose of Moderna vaccine. Has had 4 ED visits for worsening urticarial rash despite receiving treatment. Saw patient in ED tonight with rash and throat tightness concerning for anaphylaxis, responded well to medications.

Shortness of breath about 6-8 hours after injection that lasted for 1-2 hours. Used inhalers when younger, but not since then. First time having shortness of breath episode that patient remembers. Denies history of chest pain, rash, abd pain, n/v/d. Self resolved. Did not seek medical treatment.

ABOUT 1-2 MINUTES AFTER THE SHOT, PATIENT WAS DISORIENTED, STRETCHED OUT /PASSED OUT FOR A MINUTE, HAD SHORTNESS OF BREATH AND WAS LATER TRANSPORTED TO THE ER

4/23/21 at around 1pm, felt pain in left shoulder blade and around left upper lung and ribs area. Felt like tissues around it was tearing and grinding. After an hour felt pain near heart area and experienced nausia and felt faint. Was taken to hospital around 3pm. Was screened and tested for Covid-19 - negative. XRay was taken - did not show any issues Was given strong anti-inflammatory via shot Was monitored for 2 hours. Sent home. Discomfort improved over the weekend and no longer experiencing any symptoms.

After 2nd dose: On 4/21/21: shortness of breath On 4/32/21: shortness of breath, chest pain, leaning over chest pain Diagnosed with pericarditis from cardiologist treatment: Colchine for 30 days and ibuprofen for 10 days

Endorsed acute chest pain around 1200 on 6/21 initially that seemed to get better. On the early morning of 6/22 at 0400, the pain woke him up from sleep with a 7-8/10 in severity. At this point it was extremely painful for the patient to breathe but he denies shortness of breath. Denies radiation of pain, felt better when he was lying down, worse when sitting up, and was extremely bothersome while ambulating. Presented to Hospital and was found to have elevated troponin ( 7.25--13.00--2.52). Transferred to another Hospital Cardiac MRI supported the diagnosis of myopericarditis. By the time of transfer to Hospital, hos chest pain had resolved. He was treated with a 6 week course of ibuprofen and colchicine.

Patient was seated in chair when vaccinated. Patient walked about 5 or 6 steps to waiting room chair to sit down and wait post vaccination. About 90 seconds to 3 minutes post vaccination the patient was seated in chair and fainted forward. Was holding conversation with father in the next chair to him and fainted from chair to the floor. Patient was aware after about 5 to 7 seconds post fainting, arms and hands were rigid.

Upon checking on resident to obtain vital signs, resident reported "not feeling great." When asked what resident was feeling, resident stated "my body hurts all over, my arm hurts, I have a headache and I feel like my breathing is weird." Asked resident if he was experiencing SOB and resident stated yes and "it is hard for me to breath." Vital signs included BP 107/76, HR fluctuating between 122-145, O2 ranging from 95-96%, temperature 98.4, RR 19. Took resident to Emergency Department.

Today around 5:30pm, patient felt he started breathing hard, then felt lightheaded, then had tingling around cheeks, sides of trunk and arms, and then his hands became rigid. All adverse events happened around the same time per patient. Patient had difficulty moving fingers due to hand rigidity. After 10 minutes, all adverse events went away on their own and patient felt back to normal. Patient refused to visit urgent care or emergency room. Instead patient made teledoc appointment for tomorrow and was counseled by doctor if any of the adverse events return that he should promptly visit ER. Patient left after feeling asymptomatic for 15 minutes.

The vaccine makes at least for me my asthma start to flair up. It happened under the first dose too but didn't relate it to the vaccine because it was starting to get cold out and my asthma usually acts up when it gets that cold out.

Patient started not feeling well the night after receiving vaccine. Next day patient had flu like symptoms, headache, anxiety, increased heart rate and trouble breathing. Patient also presented with rash located back of neck and back - red, painful, burning and itching rash - painful to sleep on back the night after receiving the vaccine

Currently having it. I have a swollen face. Blurry vision. Difficulty standing. Shallow breaths. Headache. Chills. Body aches. 101 fever. Unable to sleep but very tired. Not able to breather through nose. Sore and numb right arm. Tingling of my limbs. Occasionally sharp pain in muscles. I got the shot at 11 and was just fine but I had work. I drove for a little over an hour and progressively my arm got weaker. I did physical labor until 10 pm and then started some school work, but found my arm almost unable to move and my mood was deteriorating. When I went to bed and lied down, it hit me. It is 4 am and it is still going strong.

Extreme fatigue, labored breathing, hyperventilation, numb fingers and toes, inability to speak, fever, backache, body aches. Was at work at time symptoms began and workplace was going to call ambulance, but instead called parents to pick him up to take to emergency room/urgent care.

During post vaccination wait time. Patient noticeably anxious/restless/bouncing legs. Initial assessment patient reported feeling like he had a hot flash and arms and were itchy. Vitals taken every five minutes, potential seizure activity noted, patient positioned for safety on cot. EMS activated, hives noted on left upper shoulder and left side of face. Patient complained of shortness of breath and crying. Patient became pale and LOC and temporary apnea noted, 0.3mg EpiPen administered in left leg x 1 dose, by RN. After administration patient gasped became combative. EMS arrived and patient transported via EMS to ED.

Seizure-like symptoms for around 3 minutes. Struggled to breath, move, talk, intense pain in chest and head. Fever and headache for several days after.

After about 10 minutes after vaccination patient complained of difficulty breathing, excessive sweating, numbness in hands and he was very pale. I called 911 and gave him one dose of epipen, Patient felt much better very quickly. Police and ems show up within a few minutes and stayed with him for about 45 minutes. He was fine and they cleared him. Neither he or ems felt it was necessary to transport him to the hospital. I called him later in the day and he was feeling fine.

9:59 AM - Vaccine 2:39 PM - Stinging pain in left arm which slowly radiates out from the vaccine site over the next several hours 7:25 PM - Pain reaches base of neck 7:29 PM - Nausea 7:30 PM - Lightheadedness 8:09 PM - Pain reaches tailbone 8:13 PM - Entire back and both shoulders begin throbbing 11:25 PM - nasal congestion 12:15 AM - body temperature drops to 97.9 12:20 AM - shortness of breath, blood oxygen drops to 95% 12:32 AM - blood oxygen drops to 92%. 12:34 AM - blood oxygen rebounds to 95% 12:35 AM - blood oxygen stabilizes at 96%. I am usually at 97-98 while resting. 1:31 AM - one of the worst sore throats I've had in my life begins 2:05 AM - low grade fever 99.1, chills, severe shivering even when under three blankets with heater on full blast, uncontrollable muscle spasms in face, arms, fingers, and legs 7:32 AM - woke up with severe muscle and joint pain. Too bad to sleep through. Can't even walk from my bed to my bedroom door without collapsing. High grade fever 101.7.

i had regular symptoms the day of and after my vaccine. Headaches, body ache, tired, sore arm, slight difficulty breathing, and slightly faster heart rate. Those didn't really concern me since i took it as the vaccine is working like it?s supposed to. but starting yesterday when i woke up until this moment i?ve had a really weird pinching feeling by my heart and the side of my rib cage on the left side of my chest. that?s the only thing scaring me now since my other side effects have already gone away.

Pt developed hives to arms, legs, chest 15 minutes post vaccine. Given Benadryl 50 mg PO with no improvement after 30 minutes. Hives continued to appear and pt c/o headache, left arm pain. Pt taken to ER at medical center. Per mother of pt, he developed SOB, tight chest after arrival at ER and was given Epipen x 2 dose and oral steroids. Admitted overnight for obs and d/c 4/25/21. On 4/26/21, developed hives again and taken with ER.

Chest pain 2 days after COVID vaccine. Midsternal cramping, pressure-like associated with shortness of breath. Pain resolved, but then returned later in the evening when lying down to go to sleep. Symptoms - chills, fever, shortness of breath, chest pain COVID 19 vaccine related myopericarditis cannot be ruled out

He started with fever and chills on the evening of the day he received the vaccine, Also severe pain in lower sternum, emesis \over the next 2-3 days developed abd pain of lower abd, cough, resp distress, waves of fatigue and hyperventilation, on 5/10 noted full body rash which was itchy, last fever was on 5/13/21, on 5/12 had blood shot eyes, 5/14 - peely skin on palms and face and felt thightness in face

Developed acute onset chest pain and shortness of breath on 5/11 with nausea/vomiting x1. He didn't have any prior systemic symptoms, URI symptoms, rashes, and was in his usual state of health. Other ROS was negaitve other than longstanding right shoulder pain without weakness.

After experiencing the expected side effects, such as fever, chills and body aches, I woke up the next day (two days after my second dose of Moderna vaccine) at 5:50 a.m. because I experienced chest pain and shortness of breath. I am concerned it may be myocarditis, and I have not seen a medical professional for this. I recovered after two days but sometimes experience pangs of chest pain time to time.

Patient received vaccine series at a separate facility. I do not have access to this information. He reported receiving Moderna dose #2 on Tuesday, June 8. Within hours, he began to have chest pain, shortness of breath, general body aches and fatigue. Chest pain was worse with deep breath and worse with supine position. On Friday, June 11, he presented to The Clinic at with these complaints. He was noted to be in evident severe discomfort, though not in respiratory distress. Vitals as follows: pulse 82 and regular, BP 139/68, respiratory rate 20, temperature 99.2 oral, SaO2 98% on room air. Exam was notable for absence of a pericardial friction rub. Lungs were clear to auscultation. he had no rashes and no remarkable tenderness of the body to palpation. Extremities were without edema. There was no sign of clinical heart failure or cardiovascular insufficiency.

Increased Heart palpitations, feeling of chest tightness, tightness In heart, pain/squeezing in heart, shortness of breath and slight vertigo. Symptoms increase when I take my Buspar or medical marijuana

Left sided chest pain, shortness of breath, exertional tachycardia, radiating chest pain to the back x 7 days. Treated with rest, Medrol Dose pack and improved over 3 days. Diagnosis from cardiolgist -myocarditis

Pt is a 19-year-old male with past medical history of hyperlipidemia, obesity who presents this hospital on 7/19/21 for chest pain and shortness of breath following administration of COVID-19 vaccination Moderna X 4 days. Patient noted to persistent substernal chest pain radiating to left upper extremity present upon exam at its worst 3/10 relieved with Advil and aspirin for which patient was taking daily following a COVID-19 vaccination Moderna first dose 7/15/2021. Patient stated pain persisted despite taking over-the-counter analgesics and had difficulty breathing today thus transported himself to the emergency department for further evaluation. Patient d admitted to subjective chills without recorded temperature, myalgias without rash, difficulty swallowing, palpitations, orthopnea, PND, increased weight gain, abdominal pain, nausea vomiting or diarrhea. Patient prior allergic reactions. Patient denied history of MI, TIA, CVA, thromboembolic events. Significant family history of mother MI and Graves' disease.

Orthostatic hypotension leading to unconsciousness, Shortness of breath, dizziness, headache, fatigue

Employee recieved the Maderna vaccination at 4pm 1/11/21. Employee was monitored for 30 minutes after vaccination and showed no side effects. He states that at 5pm he had a scratchy throat, at 8pm he had hives and his throat, chest and mouth started to itch. At that time he took Benadry and layed down. He woke up at 10pm with a swollen mouth and throat and was wheezy. He stated he had a hard time breathing and swollowing. At that time he took his EPI PEN. He did not seek medical treatment. Employee states that on 1/12/21 at 9:30am all symptoms had resolved.

Patient sitting in observation area following 1213pm vaccine administration. At 1215, 2 minutes after vaccine, patient complained of chest pain and shortness of breath. No hives or swelling of lips or throat noted. + periorbital edema. No cough or wheezing or stridor. No local reaction noted on L deltoid. Rapid Response called and pt transported to Emergency Department (ED). Cardiac workup, CXR negative. Pt given IV Benadryl and IV fluids. Symptoms resolved. Pt discharged from ED at 2:50pm same day.

Patient developed fever on day 8 post-vaccine followed by tachycardia, dyspnea and chest pain. Elevated troponin in the ER. ED echo confirmed pericarditis with likely myocarditis.

Patient reports routinely having spells of syncope with injections and still insisted on receiving COVID-19 vaccination. Gave injection into left deltoid. Had patient scoot up to table in immunization room with both legs fully under the table just in case there was an issue. I set the timer for 15 minutes and left him in the immunization room with his girlfriend, who also received a vaccine. A few moments later the girlfriend asked for our help stating he was feeling like he was going to pass out. I went into the IMZ room and checked with him. At the time, he was sweaty with a rapid hearbeat. I instructed him to take some deep breaths and went to get a cold paper towel to apply to his head. When I returned he had passed out, not responsive but still breathing, in his chair and his girlfriend was holding his head in her hand. We gently lowered his head to rest on the table while another associate called 911 per girlfriend's request. He began moaning a few moments later and was able to talk to us. He was very hot and sweaty, light headed and breathing rapidly. We applied a cooling pack to his forhead as he continued to lie his head on the table. He then said he might feel more comfortable lying on the floor. We assisted him to the floor, elevated his feet on a chair and placed his head on a roll of paper towels. At that time the paramedics showed up and said his blood pressure was fine and he was just having some hyperventilation. The paramedics got him back up in the chair and his girlfriend got him some apply juice. After about 30 minutes he was talking fine, shaking had stopped and his breathing a temp went back to normal

After recieving his Moderna vaccine the pt devoloped pallor, complained of being light headed and that his thorat felt "weird". I checked the pt's pulse, it was normal. He complained he was feeling worse, I instructed him to stop drinking water and to lay down and elevate his legs. I had a staff call 911 as the pt was complining he was having trouble breathing. The paramedics showed up before I could check his blood pressure. They checked his vitals then motiored the pt for 15 minutes. They then cleared him to leave when he felt ok.

syncope, lightheadedness, dizziness, clammy skin followed by unresponsive, shallow breathing and shallow pulse. Appeared to be in shock

Admission HPI: Right-sided chest pain began 2 days prior to admission (3/19) after getting out of car, without identifiable trigger. Pain was sharp, pleuritic, and worst with movement; didn't feel it when still. Pain not responsive to ibuprofen or heating packs. Reported 30-60 seconds of self-resolving heart racing during ocular migraine which has been his baseline for years. Not associated with SOB (at rest or with exercise), palpitations, presyncope, or syncope. No viral URI symptoms, N/V, or rash. He did receive his 2nd Moderna COVID vaccine about 1 week PTA, followed by 36hrs of fevers, chills, and body aches. Given persistence of pain, Patient asked his parents to present to urgent care. In UC, he was noted to tachycardic with CXR with bibasilar atelectasis and b/l effusions, per report. Given continued tachycardia, decision was made to transfer to ED to r/o PE. In ED, tachycardic to 104-116 and tachypneic 22-26. Had one episode of tachycardia to 182 iso ocular migraine. Endorsed chest pain but points to R middle abdomen when asked where worst. Exam was unremarkable. He received 1 dose of morphine which reduced pain from 10/10 to 0/10. Labs notable for leukocytosis (14.61), thrombocytopenia (422), elevated d-dimer (1.56), and elevated CRP (5.6). Otherwise normal chem and coags. COVID/RVP negative. CXR unremarkable. CTPA showed scattered PEs in b/l lower lobes with small pulmonary infarct in RML. No evidence of right heart strain. Heme was consulted who recommended starting him on lovenox and sending lupus anti-coagulant. Cardiology was consulted who recommended EKG, BNP, and troponin, all of which were normal and reassuring against right heart strain. ECHO was therefore deferred to the AM. The decision was made to admit. Upon arrival to the floor, Patient was tachycardic 114 and tachypneic to 22, otherwise stable vitals. He denied any pain since morphine. Denied any unexpected weight loss; reports that car ride was short. Mom reports that Patient has been less active d/t cold weather and baseball being on hold. Denies family or personal history of clots. Last seizure ~13 years ago. Hospital Course: (3/21 - 3/22) Patient was admitted to the hematology service. He was therapeutically anticoagulated with LMWH injections and his anti-Xa level prior to discharge was 0.92. (goal 0.5-1.0) Lower extremity ultrasound with Doppler showed patent veins and no evidence of DVT and upper extremity ultrasound showed no evidence of DVT, though there was potential evidence of collateralization seen in mid and distal third of right subclavian vein and left internal jugular vein. Since this was an unprovoked PE, a full thrombophilia workup was sent and will be followed up by the outpatient hematology clinic. Cardiology consulted regarding possibility of right heart strain and the tachycardia during migraine episodes. Echocardiogram performed on 3/22 was technically limited but showed no gross abnormalities. The cardiology team requested a repeat echo scheduled outpatient for 1-2 months after discharge.

Patient had the common/expected fatigue/myalgia/low grade fever following the 2nd COVID vaccine. Then, once this resolved, he developed chest pain and shortness of breath. He was admitted to the hospital and found to have pericarditis with elevated troponin and mild/nonspecific EKG changes. He had echocardiogram, CT angio coronary, and cardiac MRI as part of evaluation. He was treated supportively with NSAIDs and discharged with only mild residual pain.

Patient came to the pharmacy to get his first Moderna COVID vaccine today with his mom. After 15 minutes after receiving vaccine, he stated he felt dizziness, warmness, and flushing. I was able to elevate his feet and provide some cold water for him. After 10 minutes, he stated he felt his airway was getting closed and tongue was getting swollen. We decided to call 911 and provided Benadryl 50mg and Epipen for him. His blood pressure was 114/90 when EMS arrived. Since his vital was normal, EMS gave options to go to emergency room or urgent care. His mom and he chose to go to urgent care since he was getting better. I was able to follow up with his mom and he is going to take Benadryl for 2 days and follow up with Additional information for Item 18: his family physician. I will follow up with his doctor as well.

Patient has had swelling at site of injection for 10 days. For the last 3 nights he has been having chest pain, back pain and shortness of breath with dizziness.

12 minutes after injection - local itching/redness. 30 min. after vaccination anaphylaxis with multiple organ involvement - skin injection site and uvula swelling and eyes itchy. Patient had angioedema of throat with swallowing difficulty. Had mild asthma/wheeze. Also had tachycardia and light headed when trying to sit up. Patient given Epi 0.3 ml IM, Benadryl 50 mg IM, and Pepcid 20 mg PO. Transported to hospital by EMS for 4-6 hour observation. Patient responded well to epi.

Chest tightness and SOB is intermittent.

As with the first shot, I was very fatigued and achy. A day or two after the shot I began having pain in the left side of my chest. It was a tight feeling that made it difficult to breathe. The pain only got worse and after a couple of days, I decided to go to the emergency room as I feared I was having some kind of heart attack.

Patient mom says that son has tightness in chest when he breathes in and feels short of breath. Started sometime this morning.

6/4/2021 1730 Pt called and stated that he felt fine the day of the 2nd Moderna vaccine, but when he woke up early the next morning, he experienced dizziness and his gait was unsteady. He went to work and began to feel worse. His co-workers noticed that he was not completing his paperwork correctly and he struggled doing his daily tasks. He stated he felt like he was going to pass out and was out of breath. His supervisor sent him home. He attempted to drive home and swerved off the road. He parked his car and got a ride home. He slept for several hours. He stated he is feeling better this evening. Now he has a headache, but does not have dizziness, unsteady gait, or feeling like he will pass out. I advised him that headache is a possible side effect of the vaccine, but the other symptoms are uncommon. I asked him to seek medical attention if the symptoms come back. He voiced understanding.

Patient became pale, had chest pains, difficulty breathing, vomited, red spot formed on tip of tongue. 911 Was called. Patient was helped to the floor and a he was no longer vomiting. He was feeling better with chest pains relieved and no difficulty breathing. The red spot remained on his tongue and he was very pale. Paramedics came and he went to the hospital.

The patient reported SOB, chest tightness, sweaty, nausea, dizziness. The symptoms were reported 30mins after vaccine administration. We checked blood pressure 151/91 and pulse <100. The EMT checked vitals again and everything was in range, The patient went home with mother and she will take him to hospital if needed.

On Monday April 12 I had terrible chest pain and it was very hard for me to catch my breath. I tried to get up and walk and became very dizzy. I was then taken to the hospital and my mom stated that while in the car I had a loss of consciousness. Doctors at the hospital came to the conclusion that it was a heart attack but after following up with the cardiologist then he thought I may have myocarditis. Unfortunately I have a pectus excavatum that is being repaired and the mri couldn?t confirm the myocarditis due to the metal in my chest.

I had no symptoms after receiving the first dose, but after receiving the second dose I developed a high fever of 103 degrees as well as body aches, hypersensitivity, dizziness, migraines, and diarrhea. I eventually began taking ibuprofen to manage the symptoms, starting on night one. I then usually took ibuprofen twice a day after that. My symptoms were completely treated by ibuprofen and nearly all of them disappeared when I took it, besides the diarrhea. Oddly I also developed cold sweats when taking ibuprofen, which would dissipate after the ibuprofen stopped working. The symptoms kept coming back, as far as a week later, however, I didn?t have to take as much ibuprofen as before because they were becoming intermittent. Today, I almost got through the whole day without a return of any of my symptoms, until about 11:00 PM, after which all symptoms came back, the difference being that my fever is now low grade. This has been the case for two days now, no symptoms until night, when they all return promptly.

Within 5-8 minutes patient began sweating on his forehead and was feeling very tired. Patient was given water and 911 was called for more advanced observation. At the point fire fighters came to the seen, patient was feeling better and symptoms were almost completely gone.

per report via email - patient receive COVID vaccine during staff clinic in the AM. During the evening patient started to feel sick. symptoms included: nausea/vomiting, uncontrollable jerk like seizures, very disoriented, respirations ragged and rapid. Family called 911 taken via ambulance to local ER. per report patient received IV fluids, medicine to treat nausea/vomiting and meds to "get his respirations under control". patient sent home and is now resting.

Started with pain in the abdomen then spread upwards into chest. Serve pain around ribs making it hard to breathe. Vomited and symptoms went away for ten minutes. Pain came back gradually in same spots. This was cyclical occurance and then just stopped.

My left arm was sore starting a few minutes after the vaccine. This soreness last through that night. When I woke up the next day, my arm was extremely sore and I was very dizzy. Additionally, my pulse felt fast but my heartbeat seemed to have less strength. My breathing felt more shallow like I wasn't getting as much oxygen from the air that I breathed in as I should. For the first two weeks, the dizziness was very strong. Now, at the three-week mark, it is more manageable and life is somewhat returning to normal. I visited an ENT doctor and their diagnosis was that my dizziness was not being caused by a preexisting medical condition. Their prediction was that I would be fine, though looking back, I downplayed the severity of my vertigo. At its worst, I felt that, when I went for a walk outside, the surface was the Earth was violently spinning through space, with me stuck to the surface. It felt paralyzing. My body felt almost stuck in shock. I was afraid I would be stuck with this dizziness for the rest of my life since it was so long-lasting. Now, I am taking ginger pills and am watching my sodium intake. I feel better but am extra vigilant about taking ginger and not stressing myself too much. I do not plan on getting a second shot since some of my symptoms mirror those of anaphylaxis.

Patient fainted 5 minutes after receiving first dose of Moderna COVID-19 vaccine. Experienced low heart rate (96/53 100) and shortness of breath. Oh ok I wrote this earlier but I?ll write it again I got my vaccine at 1:15 I got there early so they have it to me then as opposed to the 2:15 appointment I felt a tightness/ lump in the throat as soon as I got it but I could talk fine and everything was fine as I was taking to people around me I left them came back at 2:15 the person in the pharmacy I had a reaction he gave me a Benadryl and I get fine and he kept me around a bit longer I?m sure he filled the information as he asked for my info and everything then at 4:57 I got some small hives on my right arm not the arm I got it on as I got it on my left arm which went away in 20mins then I went to my local hospital just to be safe and it went away mostly when I got there around 5ish I think? Then they gave me prednisone and reported it as a drug allergy I also got se. wry small hives on Thursday on my hand which went away soon after it came and I?m really hoping I can get a second dose and be fully vaccinated as it was a severe reaction

Patient developed acute onset of hives on face and body and reported facial swelling and difficulty breathing approximately 8 days after receiving first Covid shot. Stated this occurred approx. 11:40pm 3/26/21. Patient arrived at ER 1:10am on 3/27/21 where facial swelling and rash on perioral area, upper back & proximal arms was confirmed. He was given prednisone 60mg, diphenhydramine 50mg & famotidine 20mg. Home RXs for diphenhydramine 50mg po q6h prn, famotidine 20mg po bid & prednisone 40mg po qd provided. He was discharged at 02:48. Patient now recovered but will not receive second dose of Covid vaccine. *NOTE: patient stated grandmother had similar reaction to Covid vaccination which occurred one week after shot.

Patient presents to ER with chest pain, worse with deep inspiration, lying flat or leaning forward. Pain is better when sitting up straight. Vomiting x 1 episode. First Moderna shot on 4/9/21. Chest pain started on 4/11/21. On 4/14/21, pain was worse, so came to ER. Patient admitted to hospital, cardiology consult, pulmonology consult. Patient is treated with colchicine 0.6mg twice daily, NSAID and metoprolol 25mg twice daily. Follow up with cards in 2 weeks. Much improved on day 2.

First night - fever, headache, pain, chills, sweats, etc. Midnight of day 2, chest pain, shortness of breath, sweats. Went to hospital at 8:15am. Was diagnosed with pericarditis.

6 days following 1st dose Moderna, developed chills, myalgia, shortness of breath, injection site arm pain

The following morning I had a fever and felt achey overall. However later in the afternoon that day my fever had began to subside but my chest began to feel constricted. There was a constant pressure in my chest and then waves of extreme construction and pain would come and go where I felt unable to breathe and felt intense pain all over my chest and upper body.

Pt received 1st covid shot in left arm. Became wheezy and described feeling like he was going to pass out. Paramedics called to store. They observed pt and determined reaction was not from a vaccine ingredient but instead a panic attack.

After the second shot (4/21/21) I had a common reaction, fever, aches and felt really unwell the day after. The next day the only symptom I had remaining was lung/chest pain. The discomfort as not subsided any since the day after I got the shot (it had been over a week and a half), if any it has worsened slightly. It hurt most when I exhale and relax. It is a tight feeling in my mid chest.

First nausea and headache at around 6:00pm until days later. Then shortness of breathe once around 10:30. Near 11:00pm I started getting fever, chills, trouble staying asleep for more than 30 minutes. Loss of appetite for the following days. Felt like worst case of flu ever. This is a very pleasant and healthy 21-year-old with history of migraines with aura who presents to the ER 2 days after receiving his second Materna vaccine with complaint of chest pain, myalgias and fatigue. Patient reports normal postvaccination myalgias and fatigue for the last 2 days but had onset of left-sided chest pain that radiates to his upper back and is worse with deep inspiration, some positions and movement of his left upper extremity. He denies cough, shortness of breath or rash. He is low risk for pulmonary embolism with PERC score negative for PE. Screening EKG showed a sinus rhythm with elevated J point in anterior-lateral leads, ? ST elevation in 1 and AVL w questionable reciprocal changes in leads 3 and AVF. Patient went to CXR. Clinically very low risk patient for STEMI from CAD. ? Myocarditis or pericarditis? No hx of stimulant use or early CAD in family. Upon reevaluating the patient, clinically he seems to have musculoskeletal chest discomfort, worse with palpation and movement of LUE. Repeated EKG - showed more definative ST elevation through anterior and lateral leads with reciprocal changes in 2, 3, AVF. Slight ST elevation in AVL. ? Pericarditis vs myocarditis. Will call cardiac alert. Evolving EKG changes. Discussed with ED physician and Interventional Cardiology, we agreed, patient likely has acute myocarditis clinically. Very low risk for CAD. Gave aspirin and heparin. They will evaluate him. Labs coming back... wbc 17.4, normal chemistry. Sending off CRP, ESR. Troponin pending. Patient and family appreciative and understanding. 1710 - Follow up call from cardiology. Stat echo normal wall motion. With elevated CRP and mild trop 1.6, patient has myocarditis with pericarditis. Dr very appreciative for the detailed history and physical. CP free. Holding off on cath lab.

21 yo M presented with near syncope, pale, diaphoretic, difficulty breathing. EMS eval. BP 108/70, HR 165, 4-lead. Pt care transferred to responding EMS unit, Pt transported to local hospital emergency room for continued treatment.

He had Moderna vaccine #2 on 5/13/2021. The day after the vaccination he had fevers and chills. Two days after his vaccination (5/15/2021) he developed mild midsternal chest pain. This worsened and on 5/16/2021 he felt like someone was sitting on his chest. This pain was worse with deep breaths and radiated to his shoulders. He sought care at an urgent care clinic where EKG unexpectedly showed diffuse ST elevations. He was referred to the ED where he had an elevated troponin and there was concern for myocarditis. TTE done showed a normal ejection fraction and normal heart valves. He denies recent illnesses. He denies recent sore throats or skin infections. He works in the hospitality industry but denies known recent COVID-19 infection. Since admission he was seen by cardiology and was started on colchicine, indomethacin, and protonix. Telemetry has been reassuring. He reports feeling much better.

Patient presented high fever around 5pm the day of the vaccination. He continued with high fever and joint pain all Friday. He then presented with respiratory difficulties and chest pain when lying down on Saturday morning. He noticed that pain was less intense if he was seated upright. Pain became more intense around 10pm Saturday night. He was taken to am Emergency Room and was determined after evaluation that a cardio evaluation was granted due to abnormal labs and EKG results. Patient was transferred to secondary hospital. He was admitted for further evaluation and treatment.

This is a 21 year old male who had his first dose Moderna vaccine on 3/18/21 (lot#001B21A) and second dose on 5/9/21 (lot#024C21A). On 5/13/21, the patient presented to the emergency department with complaints of shortness of breath, chest pain, 'sharp shooting pain going up my leg/thigh,' pain 5/10, and nausea. Onset on 5/12/21. The patient has a history of a recent ED admission on 4/24/21 for abdominal pain, felt to be related to a recent diagnosis of hepatic stenosis, biliary sludge (unrelated to the 5/9/21 ED visit, however notable; discharged to home to f/u with established surgeon). The patient stated that this may be related to chronic anxiety. ED provider determined that acute chest pain was likely due to anxiety and/or a vaccine reaction. The patient was discharged to home with instructions to f/u if needed. No additional chart documentation available since discharge.

Pt stated that he noticed that his throat felt tight and difficult to swallow. He also stated that he was dizzy. We administered 25mg of diphenhydramine orally and monitored until a family member came to get him. He went home and I followed up with him later and he said that his symptoms had resolved

Patients mother called. Her some began having symptoms (I believe she said) the day after the vaccine. Fever, feeling under the weather. Began having chest pains and shortness of breath. He was admitted to the hospital 2 days after vaccine. An EKG was performed showing the patient was experiencing mini heart attacks.

About an hour after getting the shot, the skin on my right arm was flushing before going numb. Then my shoulder went partially numb and it's still numb/frozen as of 2:20 am 26/June/2021. Over the course of nine hours from when I got the shot, starting at around 1 hour after, I started to get symptoms, which as of right now are still here. After my skin started acting up, my blood pressure dropped and my heart was pounding extremely hard in my chest and felt tight. My muscles have been going between losing muscle tone and being locked up extremely hard. I've been dizzy since about 3:00 pm 25/June/21, and it has progressively gotten worse to where I feel like I am going to pass out if I stand up. I have chills that make my chest tighten up and cause angina. I have been feeling very faint since around 9:00pm. I've had a mild headache since around 2:30pm, and have been feeling like I am falling since about 1am. Breathing takes manual effort. I have been feeling dissociated and nauseous since about 6pm and cannot remember much from after 6pm. I am not sure whether it is a medical emergency or not but I can't afford to go to the er.

The patient received his second shot of Moderna vaccine on 6/14/21. That night he felt ill with headaches, fever and nausea. On Friday morning (6/18/21), the patient developed a 15-minute episode of chest discomfort. He described it as a moderate midsternal chest soreness radiating to his left arm at rest and worse with a deep breath, not positional. That same evening, the patient had an even worse episode of the same chest discomfort prompting his trip to the emergency department. The patient was admitted to the hospital with an elevated troponin level. He was started on colchicine and ibuprofen.

received Moderna COVID-19 vaccine on 6/28 and since 6/29 has had worsening chest pain with shortness of breath, but it was most notable yesterday. He went to urgent care , and had an EKG, with the results being abnormal. He reports it feels better when laying down and is exertional. He had chills, abdominal pain and myalgias, headache.. impression is myocarditis likely related to the vaccine.

presented to the emergency room with symptoms of chest pain. He received Moderna covid vaccine on Tuesday and since then he has been having symptoms of myalgias and chest pain. He mentions that the chest pain is substernal, gets worse with deep breaths. It gets worse when he lays on his back. And feels better when he sits up. He also had subjective fevers. When he presented to the emergency room he was having crushing chest discomfort with a troponin level at 10 and was referred for an emergent coronary angiography with concerns for acute coronary syndrome. Diagnosis/Impression: Acute myocarditis after Covid vaccination: Based on positional changes and pleuritic nature of the chest pain. Given troponin level left hand and ongoing crushing chest discomfort he was emergently taken to the Cath Lab where he does not have any significant coronary artery disease or thrombotic occlusions. LV gram shows hyperdynamic LV systolic function. At this time his symptoms are consistent with myocarditis.

The occurrence of additional non-serious events is detailed below. No Medical History information was reported. On 22-May-2021, the patient received second dose of mRNA-1273 (Moderna COVID-19 Vaccine) (unknown route) dosage was changed to 1 dosage form. On an unknown date, the patient received first dose of mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) 1 dosage form. On 23-May-2021, the patient experienced CHEST DISCOMFORT (heavy tightness in chest), DYSPNOEA (maybe difficulty breathing), ASTHENIA (weak), CHEST PAIN (chest pain) and FATIGUE (tired). On an unknown date, the patient experienced PERICARDITIS (Myopercardis) (seriousness criterion medically significant). At the time of the report, PERICARDITIS (Myopercardis), CHEST DISCOMFORT (heavy tightness in chest), DYSPNOEA (maybe difficulty breathing), ASTHENIA (weak), CHEST PAIN (chest pain) and FATIGUE (tired) outcome was unknown. DIAGNOSTIC RESULTS (normal ranges are provided in parenthesis if available): On 24-May-2021, Electrocardiogram: inconclusive (abnormal) inconclusive. No treatment information was reported. No concomitant medications were reported. On 24 May 2021, the reporter stated the patient's father noticed the patient had chest tightness and maybe a difficulty breathing, was weak and tired. The patient's father took the patient to the hospital. The patient was diagnosed with myopericarditis and was being followed by a cardiologist. Company Comment: This is a 21-year-old male patient with pericarditis and myocarditis among other events after receiving mRNA-1273. I have had heavy lungs since the day after receiving the vaccine. It is hard to breathe, and hard to get a full breath. Their is slight discomfort when I inhale.

Pt received first dose of Moderna Covid Vaccine in left arm; approximately 1345 Patient was sitting in waiting area when pt verbalized that he was experiencing swelling of the tongue and having difficulty breathing. Patient could be observed with profuse sweating. Epi Pen x1 0.3 mg IM was administered patient's right thigh, Emergency department immediately notified by medical staff and ER physician and Nurse arrive in approximately 3 minutes to assess patient. Pt transferred to ER for further care and observation.

Moderna COVID-19 Vaccine EUA The patient was in his usual state of health until he received his second dose of the Moderna COVID-19 vaccine on Tuesday 2/23. He had previously had no side effects from the first dose other than soreness at the injection site. After the 2nd dose, he noticed arm soreness at the injection site more quickly than before. 10-12 hours later, he noticed headache and subjective fever, which was relieved by Tylenol. On Wednesday he woke up with subjective fever and chills, and felt sternal chest pain during deep breathing and attempted cough, but did not notice any effect from positional changes. Other than attempting to cough to elicit pain, he did not experience any other symptoms such as runny nose, coughing, sneezing, congestion, shortness of breath, palpitations, abdominal pain, diarrhea, or dysuria. The following day, he noticed that the chest pain was more severe and persistent throughout the day. The pain location would vary from the sternal area to the left side of his chest. The intensity of pain would vary between a 2-3/10 to 7-8/10. When the pain peaked, it would be accompanied by feelings of fever, chills, and brief headache, and he felt that the pain would "spread" across his chest. The pain would persist for a variable amount of time but then eventually disappear before returning. The pain was not affected by deep breath, position, or analgesics (he took an ibuprofen at around 2 AM when he couldn't sleep). After he fell asleep he was not woken up by pain. On Friday, he pain continued to be intermittent, and peaked during a work meeting. At that point he told his boss and an MD co-worker, who recommended that he presented to the ED. The EKG that was obtained in triage showed sinus tachycardia widespread ST elevations, PR depressions, with reciprocal changes in aVR. At this time he was continuing to experience chest pain. After getting a room in the ED, he no longer felt pain. He received a 2nd EKG showing improvement in ST elevations in some, but no all leads. He received ibuprofen and he continued to feel no symptoms. He was found to have a troponin elevation of 8.84 that peaked to 11.2. Transthoracic echo showed no motional wall abnormalities, normal EF, no pericardial effusion. He was admitted to the CICU out of concern and workup for myopericarditis.

Pt received the first of the two-series COVID vaccination at 1512. Once in observation area, pt became unresponsive shortly after sitting down. This provider was called over by staff to pt, which was noted to have become unresponsive while sitting in the chair. Pt remained in the chair, did not fall or injure self. Upon arrival to patient, he was noted to have severe diaphoresis, prominent pallor, with shallow, slow breathing, no cyanosis noted. His head initially fell back and head was aligned/adjusted to maintain an open airway. This provider was speaking to the pt and performed a sternal rub to elicit a response, pt remained unresponsive and with eyes closed. Pt remained in this state for approximately 10 seconds. Pt noted to suddenly have some muscular rigidity to arms and torso and in order to prevent injury upon possible onset of seizure activity, pt was lowered to the floor by this provider and RN . Once on the ground the pt was relaxed and opened his eyes. Pt was immediately verbally responsive and asked if he passed out. Pt was able to sit up and was assisted into the wheelchair. Pt moved to treatment area where vitals and blood glucose were obtained. (@1525 BG- 104 mg/dL, VS BP 116/72, HR bradycardic @51 bpm, O2 sat 95%). Patient responses/speech was clear and pt was oriented to self, place, time and situation. Pt stated that he felt nauseated upon sitting in the observation area and admitted that he only had a coffee today, nothing to eat all day and attributed this as the cause of his reaction. Pt verbalized that he would not respond to the hospital with the paramedics and was encouraged to at least call someone to take him to the hospital. This provider spoke with the patient?s mother and explained the events. The patient?s mother stated that the patient had febrile seizures when approximately 4 years old, associated with asthma exacerbations; but pt was evaluated with a neurologist and there were no abnormal findings. Pt admitted that he has a similar syncopal episode after washing a laceration and seeing his finger bleeding. Pt states this was a few years ago and witnessed by his father that told the patient he initially fainted and then became momentarily rigid before responding appropriately (similar to today?s events). Ambulance - arrival @1536 report given and pt stated he was feeling better and declined further evaluation in truck and also declined transport to ED ( departed at 1545). Pt?s overall condition, appearance, including color was much improved. Pt denied any allergies or medication history. Past medical hx of febrile seizures at approximately age three and four, childhood asthma and one syncopal episode as an adult. Pt girlfriend arrival and she agreed to drive patient from the vaccine clinic. Pt and girlfriend were both provided with an explanation of the many concerning causes of syncope and the necessity for further evaluation in the Emergency department to rule out any underlying conditions. Both patient and girlfriend verbalized understanding and although they still declined to respond to ED directly from the clinic, both did agree to respond to ED if any changes in mental status occurred. Pt lives alone, but agreed to stay with parents and family in order to have others watch him for any changes. Pt was advised to followup with PCP and made aware that an provider would contact him tomorrow to check on his status, understanding verbalized and agreed with plan of care. Pt in no acute distress, awake, alert and oriented. Color within normal limits for and departed facility with girlfriend at 15:56, steady gait noted. Patient?s appointment for second dose was cancelled and will need to obtain clearance prior to second vaccine dose.

Around 11:30pm I started to get chills and my teeth started to chatter. At the same time my body was shaking slightly. I was laying on the couch during the start of it for the above events, this lasted for about 20+ minutes. Then I tried to get up to go to bed. I was shaking very badly at this point, teeth chattering a lot and I was very cold. I tried to use the bathroom and I couldn't even stand still to use the bathroom. My chest was also tightening and it was hard to breathe. I went to bed to lay down and I was practically hyperventilating for about 15 minutes at a time and I would stop for a minute then it would start up again. I tried to go to sleep but I couldn't because I couldn't breathe and my chest was closing in, it felt like. I called my parents in and they sat with me and gave me ibuprofen and benedryl. I started to feel better after about an hour. During that time, though, my body was extremely hot (but no fever) but I was still cold and had the chills. My head also was hurting like a headache and my eyes were really hot too. It was hard to get air and if I sat up I would get dizzy.

Pt stated he felt dizzy about 10 minutes after shot. Pt laid down for about 30 minutes. All vital signs stable. Pt still dizzy when standing after an hour. At 3:30pm, pt stated his throat "feels tight." Benadryl 50mg IM administered and 911 called. Pt transported to HCF.

Chills for 2-3 hours of duration following 5 hours after vaccination, nausea, The following morning: 10/10 non-radiating chest pain of pleuritic feeling, tingling in limbs, syncope, SOB Tachycardia(108 bpm)

after receiving the 1st dose of Moderna covid-19 vaccine, while patient is sitting for observation, patient started to feel itchy, I grabbed Benadryl syrup to administer by patient stated he feel he is going to faint and started. I supported him to lay him on the ground, while laying him on the ground I called for associate nearby for code to call for help and I grabbed Epi-pen and I was about to administer. During this time patient was still responsive and did not lose conscious. Before I administered the Epi-Pen, patient stated he was doing ok, he did not lose conscious so I did not administer the Epi-pen. another pharmacist applied a cold compress as patient was sweating. after few minutes laying on the floor patient said he was feeling better so we put him in a chair. while being seated in the chair patient stated he feels his throat is tightening and I administered 10 ml of OTC Benadryl Syrup. after few minutes patient stated his throat is getting better. Patient remained seated for 30 minutes for observation and kept applying cold compresses. After 30 minutes, patient stated he was doing better, I advised him to contact his physician and he should get the second dose after consulting with his physician and he needs to take it in a medical office

"Moderna COVID-19 Vaccine EUA" Before the adverse event, I was experiencing chills, fatigue, nausea, and a small fever. I then fell asleep. I woke up at 1:30am to brush my teeth but I couldn't take full breaths. When I was trying to take full breaths, I could only get ~70% of an inhale. This lasted for about 10 minutes. I then experienced the rest of the symptoms, minus vomitting for the rest of the day.

Around 3 pm on 4/14/21, I started to feel very tired and I started to get a headache. Around 6 pm, I started to experience lower back pain and my legs began to throb and my whole body became very sensitive to touch. I took tylenol or ibuprofen every 4 hours for the rest of the night. This went on until Thursday 4/15/21, when the dizziness and throbbing headache started. Every time I would stand up I would become off balanced and need to hold on to something. I was still experiencing the lower back pain, leg throbbing and skin sensitivity but the dizziness and headache became much more intense. Around 9:30 pm on Thursday, I felt like I was hallucinating and was experiencing euphoria along with a fever. I did not sleep the night of 4/15 into 4/16. I was sweating profusely with a throbbing headache that would cause spinning when I would stand up. Around 3 am on 4/16, I fell asleep until around 4:30 am when I woke up in a pool of my own sweat. The body aches, leg pain and skin sensitivity went away along with my fever. Throughout Friday 4/16, I had a throbbing headache and was dizzy with no relief. This continued into Sunday 4/18 when I started to feel some relief. My headache was pretty much gone but the dizziness is still very much there. I have also an experienced elevated heart rate and respirations throughout the last 5 days.

Approximately 3 to 5 minutes after immunization, patient was observed in a waiting room chair unconscious, unresponsive and not breathing (respiration was not immediately observable or recognizable). Patient was carefully and quickly placed in a supine position on the floor. Pulse was present but the patient was completely unresponsive. Respiration was not observed. EMS was immediately notified. Pharmacist and staff prepared for resuscitation and utilization of Epinephrine if needed. Patients face was flushed, arms became flushed and the patients skin became moist. After approximately 45 seconds in the supine position the patients eyes opened and he became responsive to auditory communication. Respiration became reestablished. BP was recorded at 130/91 with a pulse of 91 BPM. EMS arrived on scene and provided further evaluation. After an additional 5-10 minutes, patient was completely conscious and coherent. Patient indicated that he had a similar experience in a grocery store over 1 year ago that was unrelated to immunization and was attributed to dehydration. Patient refused additional medical care or transport to ED. Patient was released to a friend who drove him home. Patient was observed 1 day later picking up his car from the pharmacy and has recovered. Patient was advised to seek additional medical evaluation to assess the root cause of this event.

Symptoms: Extreme pain immediately after injection at site, closely followed by racing heart and winded and dizzy, followed by sweating, blurred vision and confusion. Don't remember hitting horn to alert officials. Got out of car, dizzy and stumbled. Blacked out and fell to pavement. Don't remember falling, woke up with extreme mind cloudiness, drowsiness, and hands were numb to touch but felt extreme pressure on palms. When pressure subsided, finger tips felt inflamed.

* Tuesday, March 16th, I received my first Moderna shot. No immediate symptoms, felt fine. * Thursday, March 25th (9 days post-vaccination), I was sitting at my desk and experienced some kind of visual distortion (my computer monitor bounced up and down 4 times in quick succession, even though I was perfectly still). Got up from my desk and laid down, and it went away. * Tuesday, March 30th (two weeks post-vaccination), I started experiencing persistent dizziness/imbalance. It was manageable for the first couple of days, and I still went about my daily activities. * Thursday, April 1st, symptoms got noticeably worse. I had a really bad episode of vertigo/dizziness in the afternoon. This spell of bad symptoms lasted until Saturday evening/Sunday morning. I wasn?t falling down, but I would describe it as akin to being on a boat in rough seas or a plane flying through a storm. Pressure in head and ears was intense. I?ve had longstanding tinnitus, but pitch/volume definitely seemed to change somewhat. My neck was incredibly stiff, and my throat felt somewhat ?tight? (tonsils or lymph nodes in neck felt enlarged). Took ibuprofen and Benadryl on and off over next several days. * Tuesday, April 27th, symptoms are ongoing. Still having imbalance, dizziness, and non-spinning vertigo, though less severe than previously.

Systemic: Body Aches Generalized-Medium, Systemic: Chills-Medium, Systemic: Fever-Medium, Systemic: Shortness of Breath-Medium, Systemic: Headache-Medium, Systemic: Nausea-Medium, Additional Details: Patient states symptoms began the day following second dose administration; advised patient to seek medical follow-up as symptoms reported included shortness of breath. Patient agreed stating they had already contacted their doctor but wanted to let us know as well to report the adverse effects.

Patient felt slight shortness of breath, sweats/chills, tingling in the right extremity, mental fog/fatigue, nausea within 10 minutes of being vaccinated with the first dose. Patient reported not having swelling or tingling in lips/tongue/throat. Patient reported not having much to eat or drink prior to coming in for his vaccine. Called 911 due to patient's complaints and concern about him having a possible allergic response.

22-year-old male presenting in the ED with shortness of breath following his second dose of the Moderna Covid vaccine. Patient received the vaccine on Tuesday at 1 PM. Patient states that since then he has been experiencing shortness of breath with wheezing. Negative history of the same.

Patient received a Moderna vaccine at another facility on 05/12/2021. On 05/13/2021, he had chills, fatigue, and myalgias. These symptoms resolved on 05/14/2021. However on 05/14/2021 at 10 AM he developed pleuritic chest pain. He was admitted to hospital on the evening of 05/14/2021 with dramatic elevations in high-sensitivity troponin to 1700 and EKG with diffuse ST elevations and PR depressions, concerning for myopericarditis. He was initially treated with ibuprofen and colchicine. We have ordered an echocardiogram and a cardiac MRI. Workup and treatment are ongoing.

Tightness in chest, pain in chest, and shortness of breath. Started late May 13, subsided enough to get some sleep. Awakened by the pain and shortness of breath around 7:30 am May 14. Unable to get out of bed. Called 911 for ambulance. Patient was taken to Hospital Cardiac. EKG showed ST elevation, thus L & R Catheterization was performed. Negative for MI, but diagnosed with Pericarditis, kept overnight in hospital and prescribed Indomethacin (Indocin) 3 times daily for 14 days. Released from hospital May 15. Recovery continues at home.

It started off with me becoming really irritable over the smallest things. I started getting cold chills even though it was 70F. I had a nightmare that felt like an eternity. My dreams get warped when I'm sick. I woke up and I had a headache that was centered around several spots in my head. I couldn't get comfortable no matter what I did. It's impossible to lay down on the side you get the shot on. It hurt like... 7x more than the first one when they're administering it. The scariest symptoms though, for me, was that I couldn't catch my breath. You know when you take a hot shower and you can't really breathe 100% because of the steam? That's how I felt all day after the 2nd dose. Also, I couldn't tell if I was full or not when I ate. The only way I could tell was stomach pains. That's all that's happening/has happened to me. I do feel better alot better though two days after having the 2nd dose.

Within 10 minutes of receiving vaccine, the patient became hot and sweaty, felt like pins and needles sticking and itching all over, felt as if he couldn't breathe. BP 184/143. He then began projectile vomiting. I administered and epipen (one dose) and had pharmacy staff call 911. He began feeling better within a couple of minutes, and by the time EMT's arrived, his BP back down to 118/73 and he stated he felt fine. They transported him to the ER. I called patient approx 6 hours later. He stated that he was home without further treatment from the ER and felt fine.

A patient received the first dose of the Moderna Covid vaccine. After about 10 min, he suddenly got really hot and started sweating profusely. He also reported numbness in his right arm and mild difficulty breathing. He then began to vomit. One 0.3mg EpiPen was administered and 911 was called. He began feeling better immediately and left the pharmacy with paramedics.

Per the nursing supervisor and medical records, this patient developed chest pressure, dizziness and shortness of breath 3 days after his second moderna vaccination.

22M hx asthma recieved Moderna dose #2 on 6/10. Within 12 hrs developed shortness of breath, and chest pain. Presented to this hospital's ED in this setting. On arrival he was not exhibiting symptoms of asthma exacerbation. The chest pain improved with sitting up and leaning forward.

I was sitting calmly as weekend classes were ending. I experienced a rapid heart rate followed by numbness in hands, legs and eventually face. I couldn't walk or breathe. I was transported to the hospital by ambulance and was told by the paramedics that I was having a panic attack.

ON jULY2ND-jULY 4TH FEVER, CHILLS, NAUSEA, VOMITING, BODY ACHES,. OVER THE NEXT FEW DAYS HE DEVELOPED CHEST PAIN, PALPITATIONS. CHEST PAIN WITH DEEP BREATH., LEFT HAND AND ARM WITH NUMBNESS, TINGLING

7/6/2021: Hemoptysis, ringing in ears, felt like fainting, shortness of breath -- Right Lower Chest Pain -- EMS called 7/7/2021: ER evaluation showed Acute Pulmonary Embolism --- > Admitted to hospital--- > IV Heparin --- > Xarelto. Also IV Rocephin + Azithromycin. Blood Cultures Negative x 2. EKG Sinus tach 103. Light Hemoptysis continued x 3 days. Venous Duplex Bilateral Lower Extremities Negative for DVT. 7/10/2021: Bilateral Upper Extremity Venous Duplex Negative. 7/12/2021: Discharged Home

1st Shot(5/3/21): Soreness in arm, Fatique 2nd shot(5/31/21): Soreness in arm, Fatique, headaches, developed a cough a week later which lasted about a week. Adverse event (7/6/21): Started coughing and spitting blood around 5:00pm. Coughing persisted throughout the evening. At 6:00am I told my mom I was having a problem. around 6:30am my body went numb, loud ringing in my ears, and broke out into a cold sweat, so we called 911. The paramedics arrived and checked me out, and they believed I was okay. When they left we decided to go to the ER. Admitted on 7/7/21-7/12/21

Patient experienced a hard time breathing while in the waiting room, after receiving his first dose of the Moderna COVID vaccine. He approached the pharmacy counter and was gasping for air. Pharmacist and tech helped him to the waiting area so he could sit. Upon quick evaluation, patient was pale, sweating and panting. Pharmacist immediately gave him epinephrine 0.3mg injection, and the store manager called 911. After a couple of minutes, patient was able speak and breathe easier. Patient regurgitated and stated he felt much better. The ambulance arrived 15 minutes afterwards and the paramedics checked his vitals (good pulse and blood pressure). Patient was then transported to the local hospital for observation.

Patient made statements that he was having a harder time swallowing. Blood pressure was taken. We assessed and then he began to report difficultly breathing and a very difficult time swallowing. Blood pressure was taken and lung sounds assessed. EMS was called. Epi pen was given. Within 5 minutes he reported feeling much better and no issues with swallowing or breathing. EMS arrived on site and he was transferred to the local hospital.

Had vaccine on 1/7/2021 afternoon, the next morning and Saturday and Sunday developed difficulty breathing, dizziness, fatigue, headache and went to Medical Center ER was evaluated and a COVID test was done. Results pending. No blood work, CXR, or EKG was done. Patient reports symptoms are now intermittent seem to be improving. Patient will check on hospital website for COVID results

Patient came back from the unit after returning back to work stating that he "felt like he had run a marathon". Upon assessment, patients vital signs were BP 152/110, P 83, O2 sat 99% although patient stated that he felt "short of breath". Patient reported a history of anxiety and HTN, but was not taking medications for blood pressure as he had not "picked them up". Doctor assessed and called EMS. When they arrived, they did ask him if he wanted to go to which he replied "it would probably be best". No further complaints or treatment reported at this time.

Approximately 10-15 minutes after receiving dose of covid vaccine case reported he had the sensation is throat was "a little sore and if felt like it was beginning to swell. No observed obstruction in airway. Had own epi-pen and this supervisor also had available but he declined at that time. Consented to oral elixir of bendryl 50 mg po which was given. He reportedly felt tachy Pulse rate 96. Case reported last time he needed epipen he felt pain like needles in throat area. Continued to observe. Case reported sensation of swelling was dissipating within 15 minutes. Continued to observe an additional 40 minutes. Had case call for ride home since he took bendaryl. Also had case call his physician to notify him of reaction. Case departed clinic about 11:50 breathing WNL. Girlfirend escorted him home. Instucted to call 911 and administer epipen if symtpoms worsen or return. Contact caste 5:30 pm at home he reported he has continued to improve. No throat swelling, but has slight sore throat. He will continue to have girlfriend stay with him. His physician called back and provided the same instructions.

Moderna COVID-19 VACCINE Fever, chills, cough, sweating overnight and then loss of taste and smell the next morning. Soreness at the site of the injection. Extremely fatigued. Tmax 100.5 Bodyaches, Headaches , Nasal congestion, Shortness of Breath, Chest Pain. Four days after 2nd Moderna vaccination his loss of taste and smell is still affected. PT EXPERIENCED NAUSEA, HA, FATIGUE 1 DAY POST VACCINE. REPORTS HE FELT BETTER 2 DAYS POST VACCINE AND WENT FOR A REGULAR WORK-OUT WITHOUT COMPLICATIONS. PT WOKE-UP 3 DAYS POST VACCINE WITH CHEST PX, SOB, FATIGUE. HE CALLED 911 AND WAS BROUGHT TO HOSPITAL VIA AMBULANCE. PT HAD UNREMARKABLE EKG INITIALLY, LABS DRAWN. INITIAL TROPONIN 4.7, EKG REPEATED WITH SOME MINIMAL ST ELEVATION, 324MG ASA GIVEN. COORDINATING TRANSPORT TO ANOTHER HOSPITAL WITH CARDIOLOGY SERVICES, TROPONIN REPEATED AND FOUND TO BE 10.8 WITH GLOBAL ST ELEVATIONS ON EKG. PT TRANSPORTED TO HOSPITAL FOR PRESUMED PERICARDITIS. AWAITING UPDATES ON PT CONDITION TOMORROW.

patient felt like throat was swollen while walking around. he was able to stand, talk and breathe without difficulties. monitored patient while paramedic arrived. paramedic did a scan of the patient and offered to take him to hospital for follow up but patient denied going with them

Patient (nursing home employee) was given his second dose of the Covid 19 vaccine today (March 24,2021) during a routine nursing home vaccination clinic. Patient was given his second dose at 8:35 am and at 8:39 am he was complaining of shortness of breath, chest pain, as we as pain in his extremities. Patient stooped in chair and lowered to the ground. Epi Pen 0.3 mg was administered by the attending pharmacist, at 8:41 am and the ambulance was called. HIs vital signs were taken: Blood pressure 123/77, Oxygen 99%, pulse ranged from 104 to 111 bpm. Patient complained of pins/needles feeling in extremities, his arms (both left and right) were contracting, and his pupils were dilated. He was given 4 L oxygen, and the ambulance arrived at 8:58 am.

Lightheaded and trouble breathing. Pt was anxious. Had some water, rested with elevated legs and felt better. Ambulated out of facility without assistance

The patient experienced body aches and headache within the first 24-48 hours of receiving the vaccine. On 4/1/2021, the patient had swelling around his eyes at around noon. He stated that he feels that the swelling worsened that night but this morning (4/2/21) the swelling has improved. He came to the clinic this morning to check in regarding his symptoms. I called in PA to assess the patient. She assessed the patient and he stated that he had some SOB yesterday but denied any SOB today. The patient's O2 sat was 99% and HR was 98 bpm. She recommended that the patient take Claritin BID and return for follow up on 4/5/21 to see her in clinic. She advised him to go to the ER if he experiences any SOB, increased swelling, or difficulty breathing and he verbalized understanding. The patient has a hx of HTN and takes Lisinopril 10 mg daily. He did take his medication this morning. The patient denied taking any new medications. He also denied any previous vaccine, drug or food allergies.

PATIENT RECEIVED THE FIRST DOSE OF MODERNA VACCINE WITHIN FIVE MINUTES PATIENT WAS DIAPHORETIC, LIGHTHEADED , AND HAD COMPLAINTS OF SHORTNESS OF BREATH. PATIENT WAS TRANSPORTED TO ER PER WHEELCHAIR PATIENT REQUIRE BENADRYL, IV FLUIDS, AND SOLUMEDROL. PATIENT REPORTED SYMPTOMS RESOLVED AND PATIENT WAS DISCHARGED FROM ER AND INSTRUCTED TO FOLLOW UP WITH PRIMARY CARE PHYSICIAN IF NEEDED.

PATIENT STATED HE WAS FEELING REALLY HOT AND DIZZY. PATIENT WAS HOT TO TOUCH. PATIENT THEN STATED HE WAS HAVING DIFFICULTY BREATHING. I HAD HIM LAY DOWN AND ELEVATED HIS FEET. KEPT TALKING TO HIM AND ASKED HIM IF HE FELT LIKE HIS BREATHING WAS GETTING WORSE AND HE SAID YES. I ASKED TO CALL 911 AND I ADMINISTERED ONE DOSE OF AN EPI PEN INTO HIS RIGHT THIGH. WITHIN 30 SECONDS I ASKED HIM HOW HE WAS FEELING AND HE SAID HE COULD BREATH BETTER, HIS SKIN STARTED FEELING COOLER TO THE TOUCH. PARAMEDICS ARRIVED AND TOOK HIM ON TO THE EMERGENCY ROOM.

Minor cough and minor shortness of breath. No change in symptoms after using previously prescribed albuterol rescue inhaler.

23 YEAR OLD admitted to hospital with chest pain and elevated troponin (5000). Had received second dose of Moderna vaccine 4 days prior. Myalgia and fever from vaccine were resolving when chest pain and dyspnea started. Normal angiogram, no coagulopathy, normal platelets, normal echo. Pain resolved.

Swollen eye lids (top and bottom), rash on face, hands, necks. Itchy all over. Throat tenderness and tight feeling

I took the FIRST dose of the Moderna vaccine this Tuesday on the 20th of April at 11:30 AM. Around 10:00 PM Wednesday I have been feeling short of breath. This feeling has persisted throughout the entire day of Thursday and I continue to feel the same way (I think it has slightly worsened). I can breath, but I am completely unable to take a full, deep breath. I sometimes can achieve a full deep breath but it's completely at random. I find myself trying to forcefully take a deep breath but can't seem to fully fill my lungs how I normally am able to. Additionally, my throat feels mildly swollen/sore, starting from the back of my mouth and down my esophagus. The lowest point of my esophagus at which I feel the soreness/mild swelling is near the center of my chest. I think my shortness of breath is progressively, and slowly worsening, but its hard to tell for certain. Some of the muscles around my back are beginning to feel uncomfortable due to me constantly trying to forcefully achieve a full and deep breath.

Pt received first dose of Moderna covid vaccine and during observation period of 3 minutes, pt stated "I feel funny in my chest and a little shortness of breath." VS taken= P-102, BP- 149/98, RR- 20. Skin warm and dry. No n/v noted. No c/o AP or numbness to hands or legs. Mother with pt. Notified lead RN, LVN of situation and wheeled downstairs to facilities urgent care.

Systemic: Allergic: Difficulty Breathing-Medium, Systemic: Allergic: Difficulty Swallowing, Throat Tightness-Mild, Systemic: Chest Tightness / Heaviness / Pain-Medium, Systemic: Dizziness / Lightheadness-Medium, Additional Details: Approximately 5 minutes after getting Moderna Covid Vaccine, pt complained of chest pain, throat tightening and shortness of breath. I Administered Generic epinepherine auto injector to his right thigh, and within minute or two, his breathing normalized but pt still had throat tightness. EMS arrived at that point and examined the pt and transported him to local hospital.

Patient presented to ER 4/26/21 with sharp chest pain associated with SOB that started the night of 4/25/21. EKG suggestive of pericarditis. Troponin I peaked at 11 and trended down to 6, TTE showed normal EF, no structural abnormalities and no pericardial effusion. patient was observed overnight, did well, discharged 2nd day on Ibuprofen 600 TID PO for 2 weeks. chest pain persisted for two more days and now completely resolved off the NSAID.

23 yo man in high school when standing, admitted wit acute onset epigastric abdominal pain after a run, found to have mild elevation in troponin, flat at 0.04, dynamic EKG changes admitted or work-up 1. Acute epigastric abdominal pain/elevated troponin: The patient was chest pain free from the time of treatment with Pepcid, magic mouthwash, and Toradol in ED. His echo is without wall motion abnormalities, troponin is flat and remains chest pain free. The patient had a CT PE protocol study and CTA coronary without acute findings. He had a cardiac MRI which showed no acute myocarditis but may have had some delayed contrast enhancement and may represent prior myocarditis. He was started on low dose lisinopril 2.5 mg po daily for cardio-protection, will reduce physical activity with slow return to activity, and follow with cardiology with an urgent exercise stress test as an outpatient. His u tox shows cannabinoids and no other intoxicants, His lipid panel wnl, HDL 65, LDL 84, His TSH is wnl. His CRP and ESR is wnl. 2. Left axillary lymphadenopathy: Discussed with patient lymph nodes seen on CT scan in left axilla and left pectoral region. Patient cofnirmed he had a COVID 19 Moderna vaccine in the left arm on 4/3/2021. Epic documentation notes in the right axilla, but patient is clear that he had the vaccine in the left arm. Axillary lymphadenopathy is not surprising in this situation. 3. Small 4 mm nodules noted on CT scan: The patient had no infectious symptoms, no cough, no fever, and no elevated WBC. He does smoke marijuana but has no tobacco use. He can follow with PCP to further risk stratify the nodules, either with PPD, and consideration of repeat scan in 6-12 months as needed. 4. Mild bilateral gynecomastia: I informed patient this has been found. He is not bothered by this finding and it seems an incidental finding. Moderna COVID-19 Vaccine EUA Symptoms: Muscle tenderness at vaccination site, general achiness, fatigue, occasional difficulty breathing (felt like someone squeezing on my diaphragm and made it uncomfortable breathing for short periods of time. Started in the evening time, lasted a minute max each time, and occurred around 5 times before ceasing). Symptoms ended around 9 pm the same day they began

Patient have 2nd Moderna vaccine administered at 2:02 pm . At 2:30 pm patient reported an itchy skin, vague tingling and mild throat closing. Patient was accessed by P.Bergen PA and Benadryl 25mg was administered per his order. Patient was sent to ED via ambulance.

Soreness, chills, 101.5F fever, loss of consciousness, difficulty breathing, pain in lungs. Worsened over two days, then recovered on third.

Patient started having generalized weakness, fatigue, fever and headache after the second dose of his Moderna Covid-19 vaccine. These symptoms persisted throughout the next day. However, two days after the second Covid-19 shot patient developed severe chest pain that resulted in presentation in the ED. There was associated shortness of breath and diaphoresis. He was treated with Colchicine and Ibuprofen. He was also started on metoprolol because he had an 11-beat run of non-sustained ventricular tachycardia. He was discharged home on the fourth day to follow up with cardiology as outpatient.

At approximately 3:30 a.m. on 04/19 I experienced strong pain in the chest, armpit, and upper back/shoulders accompanied by discomfort breathing that lasted 2.5 to 3 hours. These symptoms faded and altogether went away between 6:30 and 7:00 a.m. At 9:00 a.m. I went into prompt care and had the following tests and procedures as described in Item 19.

Shortness of Breath, fever, chills, "blacking out," difficulty breathing, dizziness, nausea, vomiting, weakness; starting after vaccination approximately 40 minutes. patient was at home at symptom onset

Chest pains beginning around 48 hrs after vaccine administration and ending around 2 weeks after receiving vaccine. Shortness of breath at night while sleeping for approximately two weeks after receiving vaccine.

Heart inflammation (diagnosis myocarditis from one hospital & pericarditis by the other) Chest pains, trouble breathing, cannot move easily without pain., heart palpitations (those ended). Started off being told to take Advil every 6 hours. That didn?t help, went to a new ER, they gave me colchicine. It?s helping but I still am pretty much bedridden.

About 4 days post 2nd Moderna vaccine, patient started to develop shortness of breath, chest pain, and night sweats.

Patient became nauseated at 11pm 1/20/21 and fever of 101, passed out in bathroom dry heaving and woke up at 4am, Heartrate 182, short of breath, continuous vomiting, called EMS, was given zofran on route to ER, upon arrival to ER heartrate reduced to 130's, patient reports being given toradol and IV Fluids, and was released. Patient reports fever range of 101-103.

Reported that within 2 minutes of receiving vaccine,began to experience a swelling feeling in his throat.

24y/o male with remote history of Kawasaki disease fevers, chills, night sweats, anorexia, nausea, vomiting, dehydration, chest pain, dyspnea, constipation. Symptoms began around 1600 on 27JAN21 (day of immunization). Worsened on 28JAN21. On 29JAN21 presented to primary care with tachycardia prompting IV fluid resuscitation.

Moderna COVID-19 Vaccine EUA employee received moderna vaccine on 1/13/21, Lot number 011L20A, Exp 07/03/2021. Patient started experiencing discomfort in the chest and it hurt when he breathed, cough and sneeze. 28 days today he is still having the same symptoms. we advised employee not to get 2nd dose and referred to employee health

Patient was given dose one of Moderna vaccine this morning. Patient returned to our site around 1230 pm with ?facial swelling.? He also reported throat itching and tightness.

Patient had reported onset of symptoms 03MAR21 @1900 following 1st Dose of Moderna on 02MAR21. Patient reported to Emergency Department at 0000 04MAR21 with the symptoms of Vertigo, Lightheaded, Nausea, Chest Tightness, and SOB. EKG, UA, and Chest X-ray ordered by ED Provider. ED Provider notated that "CXR and ECG Normal". ED Provider diagnosis: "Acute Pleuritic Chest Pain, Dizziness, Immunization Side Effect". Patient received APAP and Ibuprofen in ED and reported that chest/pleuritic pain improved following administration but complete resolution did not occur for another 24hrs. Patient discharged from ED on 04MAR21.

3/11/21: patient presented and was admitted through ED for Shortness of Breath and choking on food. His saturation was as low as 70s and placed on nonrebreather. Patient was brought to ED and eventually required to be intubated because of hypoxic respiratory failure. During hospitalization patient has one episode of seizure. Patient had similar presentation in May 2019 and he had to be intubated for choking fluid. per the EUA, hospitalizations to be reported irrespective of attribution of vaccine.

Per RN, "Patient here with worker group to receive 1st moderna COVID vaccination. Patient received vaccination approx 1436. At 1446 patient had complaint of dry lips and statement that he has allergy to something but unable to recall. Pulled patient to recliner chair and using interpreter asked about symptoms further. He then stated that his throat was feeling like it was tightening up and swelling. At 1448 administered emergency epi pen dose to right outer thigh. Patient was then rolled into building and presented to the emergency department for evaluation. Within couple minutes of the epi shot, he stated that he was starting to feel better. That statement was as we were rolling down the hall towards the ED." Patient taken to the ER to e seen from 1450-1620. RN in the ED, notes throat and mouth dryness while in the ED. Patient was monitored without complication and discharged home with employer representative.

Adverse events: 2:05 p.m. Erythema, flushing, tightness in throat and chest, flushing Treatment: 2:10 p.m. EpiPen administered and call to emergency line

Patient after the vaccination during the waiting period started having vision changes, dizziness, shortness of breath, itching, paleness, sweating, throat a little tight. 911 was called, and patient monitored very close with Epipen out and ready to use if needed. 911 team arrived to store patient had some water and was starting to feel better. Patient ended up feeling fine and it was thought after all tests to be an anxiety attack. No medication was given. Patient did not go to ER.

Patient was scheduled for his first Moderna vaccine on Friday April 23rd at 3:00 pm. Intern administered the vaccine. After receiving the vaccination patient was instructed to wait in the designated waiting area for 15 minutes. After 5-10 minutes, patient walked over to the drop-off window and stated that he was having a hard time breathing and his hands were going numb. Pharmacist on duty, immediately left the pharmacy and walked over to the patient. The patient then fainted in his arms and pharmacist was able to gently bring him to the ground. Patient wanted pharmacist to call and notify his wife which at the time was waiting in her car. His wife arrived immediately. Technician, called 911 and ran EpiPen over to pharmacist and technician prepared a damp cloth and placed it behind patients neck. Pharmacist then administered one dose of adult Epi-pen. Minutes later patient still said he could not breathe and that his symptoms were getting worse. Pharmacist then administered second dose of Epi-pen. Within 3-5 minutes after receiving second Epi-pen dose, the ambulance came. EMT's gave the patient oxygen and placed him on a gurney. EMT's then took patient to ambulance and drove him to hospital. Going through patient's immunization consent form and in talking to patient, there was no indication that the patient has ever had any reactions to any vaccinations in the past.

Patient vaccinated at 1035 - was in observation area for 30 minute observation. Patient reported to observation RN s/sx consistent with history of his anaphylactic allergy to shrimp - i.e. "throat closing" patient reported not feeling like he needed to get epi yet and did not want it. RN lead notified. Patient rapidly assessed as pale, slightly anxious but reserved. Patient was agreeable to taking 50mg Benadryl PO - given with sips of water. VS BP 140/80 P 92 Spo2 98%RA. - Stat team initiated - pt rapidly assessed and transferred to ED per patient request. Chart review indicates patient medicated in ER with Solumedrol, famotidine and loratadine and given a bolus of 1L of NS. VS remained stable throughout. Discharged with 2 days of 20mg Prednisone PO daily. Pt has supply of epi pens at home.

Patient presented to Clinic with Allergic Reaction Symptoms, including shortness of breath, Chest tightness, and Nausea. Patient states he had gotten the shot roughly 1 hour before coming to Medical. Patient reported to Medical at 1038, where vitals were immediately taken. Patient Vitals were as follows: BP: 148/92 Temp: 98.1 Pulse: 90 R: 20 SPO2%: 100 EMS was called at 1052 and had arrived at 1102. EMS had left with patient at 1106. Patient did report a history of Childhood asthma, no allergies to medications.

Chill, tiredness and shakes right after the shot. There days after I got difficulty to breath at my wake up and lasts around 2 hours

On Sunday April 25th 2021, I woke up with severe chest pain that went up to the right side of my neck, as well as having shortness of breath. I was admitted into the ER and they ran blood work, did a CT scan and x ray of My chest. When my bloodwork came back, it showed I had very elevated troponin levels. Because I was having continuous pain, I was given morphine but it didn't work. So they upped it and gave me dilaudid and that took the pain away. It was in the doctors best interest I get transported to a facility with a good cardiatric team. Upon arriving to the new facility, they further drew my blood and ran more tests. I got admitted as an in patient on the cardiatric floor. The cardiologists ordered for me to have an echocardiogram done, another CT scan with a special protocol to view my heart, and an MRI done of my heart. After my echocardiogram and CT scan came back fine, the doctors said they'd be surprised if the MRI DIDNT show something. When that result came back, it did reveal my heart was still inflamed. After ruling out all other possibilities they concluded that the 2nd Dose of Moderna COVID vaccination I'd received was the culprit behind my diagnosis. My diagnosis being, a case of myocarditis as well as a mild heart arrhythmia. For the next several months I am kn a medication called metoprolol 25mg, 2 half tablets a day, to correct my heart arrhythmia and relieve stress from my heart as well as minimize the amount of stress I encounter each day. No heavy lifting pushing or pulling of any sort for the next 3 months.

Pt developed right throat tightness 30 minutes after receiving vaccine at 2:40 pm. Pt was in no acute distress, conversive, and vitals WNL except BP elevated 153/90. Lung sounds clear throughout. diphenhydramine administered via IM injection and patient was observed until 3:50pm.

A few seconds after shot, pt became pale and immunizer asked if he was ok. His muscles tensed and shook a little and he passed out, falling slightly to the right. The technician was able to place herself between him and the floor and slowly lower down, leaning against the wall of the pharmacy. By this point, about 30 seconds at most had passed, and pt was awake and cognitive, but had no recollection of passing out or how he had gotten to the floor. He did know his name and where he was, but was breathing heavy. We took his blood pressure at this point and it was 82/46 with a pulse in the 40's as well. Pt refused ambulance and we had him continue to sit on the floor. Patient mentioned he had this type of reaction in past to other vaccines (not stating what), though only telling us prior he had "slight dizziness once". He also stated he had not had anything to eat or drink that am so we got him a snack and some water. Patient continued to breath deeper as we suggested and stay on the floor as we called mgmt from the store. We took blood pressure about 10 minutes later and it had gone up to 117/71. and then again 10 minutes later and it was 121/69. After 30 minutes total, pt asked if he could stand and leave. He stood with no dizziness and said he felt "fine". We called patient approximately an hour later at his residence to make sure he got home ok and patient stated he had no other symptoms at that time and continued to feel "normal".

He got the vaccine, mom received her shot and the pharmacist said they were done, and he told his mother that he was feeling a little dizzy and anxious and wanted to sit down. They were going to the car and he got in first as he was going to drive, and mother got in the car, and he acted like his head was spinning, had a blank stare and started making weird sounds like he could not breath, started foaming at the mouth and then he passed out. She called 9-1-1 and when they got there he was coming out of it. They checked his VS and said that everything was OK, and felt that he did not need to go to the hospital but it was their call. He didn't want to go either. She drove him home and he slept OK, he had a bad headache and felt extremely tired and lied down. He seemed to be OK the next day other than his arm being sore, but no other issues and he's been fine since. The pharmacy did not have them wait before leaving to make sure they were OK, the pharmacist told her they were fine and they could go, but the incident occurred within 5 minutes of the vaccine.

Severe flu-like symptoms. Woke up at 4am with a splitting headache, my ears ringing loudly, and my whole body aching like crazy. Severe dizziness any time I stood up. Slightly short of breath. High fever and chills. I?ve been taking ibuprofen every few hours and drinking lots of water, and I have felt better as time has gone on. Right now I?m still slightly achy but I don?t have a fever anymore and the dizziness and chills are gone. My appetite is starting to come back too.

The afternoon following the vaccine (5/7/21), he began to have a hard time breathing, at work, switching breathing between mouth and nose and felt warm. He is a dishwasher. By 5:00 pm, both of his arm muscles and stomach was cramping. He was having bigger problems breathing, and becoming woozy. The paramedics were called to his work, and he was transported to the ER as they felt he was having an allergic reaction. He had an IV, Tylenol for a fever, Atavan to relax, Benadryl for antihistamine as they could see his arms were red. The ER did urine and blood tests as well as a Covid test. He was also given oxygen to bring his oxygen levels back up

At 9:10 PM on 5/11/21, I experienced chills that required multiple layers of heavy blankets. This lasted till about 10:20 PM on 5/11/21. I was shaking a lot. I never experienced chills that extreme before. At 10:30PM on 5/11/21, I took one Tylenol. At about 2:00AM on 5/12/21, I woke up with heat flashes for an unknown time due to sleeping. At 7:30AM on 5/12/21, I woke up to a heart rate of 105 beats per minute. This lasted for about six hours. I could feel my heart pounding through my chest. During this event, I also experienced shortness of breath. That lasted for about the same time. At 9:00AM on 5/12/21, I took one Tylenol. By 4:00PM on 5/12/21, I started to feel better and my heart rate got down to my normal level of 70 beats per minute while resting.

WHEN I CAME BACK PATIENT WAS RIGID AND LOCK JAWED AND NOT BREATHING. WHEN WE GOT PATIENT TO FLOOR HE WAS RESPONSIVE TO A STERNUM RUB AND HAD A PULSE. AT THIS TIME HE STARTED TO PRODUCE SNORE LIKE NOISES, CONVULSED FOR LESS THAN A MINUTE THEN CAME BACK TO. EMT'S CAME AND CLEARED PATIENT. PATIENT STATED HE HAS A VASOVAGAL REFLEX HOWEVER HAS NOT HAD AN INCIDENT IN MANY YEARS. IT IS UNKNOWN IF THIS IS WHAT PATIENT EXPERIENCED OR A REACTION TO THE VACCINE.

After 2 months I started getting heart palpitations, shortness of breath and chest pain. This is months after vaccination. Vaccine was Moderna. Both doses taken.

Patient arrived via ambulance to the emergency room with shortness of breath, worsening left sided chest pain, and fever. Patient had history of chest pain over the last 3 months since getting covid in feb, but reports this pain is different. Treatments include EKG/CXR, labs, aspirin chew 324, acetaminophen 1000 mg IV, and ibuprofen 800. Advised to follow up with primary care provider and rest for 48 hours. Patient was discharged home.

24-year-old male presents with nondescript anterior chest pain and shortness of breath that is worse with breathing. He states this started acutely this morning. He reports 3 days prior he got his second dose of Moderna Covid vaccine. He reports a sensation of chills as well. He denies any fever, abdominal pain, exertional shortness of breath. Reports no cough or hemoptysis. Echo more procedure the patient's troponins were significantly elevated and had diffuse ST elevations throughout. Was given aspirin and Toradol with no relief. Was transferred to Hospital for further evaluation. Patient was evaluated by her interventional cardiologist and had a bedside echo obtained there is no wall motion abnormalities and this was like bleed thought to be secondary to his recent Covid vaccination. Will be admitted with a diagnosis of pericarditis for monitoring overnight.

On an unknown date, the patient experienced NEUROMYELITIS OPTICA SPECTRUM DISORDER (neuromyletis optica) (seriousness criteria death, hospitalization and medically significant), ASTHENIA (weakness) (seriousness criteria death and hospitalization), DYSPNOEA (difficulty breathing) (seriousness criteria death and hospitalization), HYPOKINESIA (unable to move his legs) (seriousness criteria death and hospitalization) and LUNG DISORDER (Lung problem) (seriousness criteria death and hospitalization). The patient was hospitalized on 09-Jun-2021 due to ASTHENIA, DYSPNOEA, HYPOKINESIA, LUNG DISORDER and NEUROMYELITIS OPTICA SPECTRUM DISORDER. The patient died on 08-Jul-2021. The reported cause of death was Neuromyelitis optica. It is unknown if an autopsy was performed. No concomitant medication was reported. Approximately 12 hours after injection, I experienced hot flashes, chills, and a severe headache. The next morning I experienced a less severe headache but experienced noticeable shortness of breath especially on exertion. Tylenol P.O. helped my headache that morning, no known fever. Approx 2 days after injection I also experienced a noticeable reduction in the ability to smell or taste. This Stayed for at least another day or so.

After receipt of the vaccine, I developed a low grade fever the next day (1/7/21) that persisted throughout the day until it progressed into a fever of at least 101.3 F at night. It might have been higher, but that was the best reading I could get. It felt like the fever came on extremely fast bc all of a sudden I started shaking vigorously with rigors and chills. I couldn't get out of bed without experiencing the most uncomfortable chills I've ever experienced. I previously had Covid in early November, and this fever felt worse than what I experienced when I actually had Covid. Also, I experienced moderate to severe pain in my left arm on 1/7/21 at the injection site, which was far worse than any other muscle soreness I've experienced from any other vaccine I've received. The hour preceding the sudden fever attack, my left arm was feeling blazing hot, looked a little red, and swollen. I also experienced a strange allodynia type pain, whereby parts of my body hurt from simply touching them. Although I did not develop a significant rash, I experienced pruritus all over my body, predominantly affecting the areas around my ankles and wrists. I have a hx of multiple dermal filler injections in my lips. I don't think I had any problems with my lips after the injection. But other symptoms that I developed included a tickle in my throat and some shortness of air. On 1/8/21, I woke up drenched in sweat so I presumed that I broke my fever while sleeping. I have felt much better ever since. I'm a nurse practitioner student, and I had to miss a day of clinical on 1/8/21 bc of this reaction to the vaccine. I used ibuprofen the morning on 1/7/21 and Tylenol at night on 1/7/21 to bring down my fever

The location of the injection is very sore. I'm also experiencing mild shortness of breath and fatigue.

Patient received his 2nd dose of the Moderna vaccine prior to hospital admission at unknown site. Patient reported generalized body aches and that his whole body hurts and feels likes he's "being poked with a knife." He endorsed associated symptoms of fever, nausea, back pain, and shortness of breath. The patient denies any sore throat, headache, cough, or vomiting. His back pain is at the tip of his shoulders, in the middle of his spine, and near his kidneys. The patient adds the abdominal pain is worst in the lower left quadrant. Patient admitted for bilateral lower extremity weakness limiting him to ambulate and to rule out Guillain-Barré. During admission, had fevers, photophobia, phonophobia, and nuchal rigidity. On physical examination patient was able to minimally wiggle toes, had no plantar/dorsiflexion, unable to move hip joints or knee joints bilaterally, lift his legs against gravity.

Almost 12 hours post-injection, I experienced a rapid onset fever, soreness, muscle aches, dizziness, high blood pressure, headache. Upon awakening the following day (today, 20 hours post injection) severe allergies around eyes. 24 hours post injection, all day today, fatigue, severe dizziness, soreness, headache, high blood pressure, loss of appetite, and labored breathing.

Anxiety/panic attack after- issues catching breath, hyperventilating, fast HR rate but WNL- Forgot his inhaler at home. Had a family member pick him up and take him home for the day- advised him if symptoms persist, immediately to the ER

Pt began to feel sweaty with sensation of throat tightness about 15 min after vaccine was injected. At that time, all vital signs normal. Pt did not have tongue swelling and did have a patent airway. No rash. Patient was given a total of Benadryl 50mg and Pepcid 20mg. He was observed for 1.5 hours, during which his vitals remained normal, airway remained patent, and no rash was noted. Patient continued to have sensation of throat tightness after the observation period, and we recommended that he proceed to the ER. He declined to do so. He was then given a syringe of epinephrine 0.3mL of 1:1000 solution for the ride home. He was driven home by his parents. We attempted to contact him later that evening but received no response.

COVID-19 Vaccine EUA. Painful/sore at site of injection; left arm approx. 2-3 hours after injection. Also within 2-3 lower back pain/soreness. Night of vaccine, shortness of breath, full body aches/soreness, fever, fatigue, headache, chills, and nausea which followed into next day (today).

Patient developed throat swelling, chest tightness, dizziness, and headache ~30 min after vaccine administration. Benadryl 50mg was given (11:00am) , throat swelling continued and Epi pen was administered 35 min afterwards (11:35am). Advil given at 11:50am. Vitals remained stable

I started feeling lightheaded and dizzy. They kept me for an extra 30 min. They brought me a chair to put my legs up. My heart started beating fast. I couldn't stand up and was scared to walk, so I was put on a stretcher. My throat started to close up. I was put in an ambulance and my throat swelled more on the trip to the ER. They gave me Benadryl and Prednisone. I was there for about 4 or 5 hours. I was okay after that. I checked the CDC site and it said I couldn't take the 2nd dose.

0927 vaccine dose 0947 reported diffuse itching and "felling like I do when I eat nuts" 0950 BP 120/74, HR 100, SpO2 99% (CTAB, no SOB, no rash or oral swelling) 0952 reported he felt his "throat closing" the same as prior episodes of anaphylaxis so epi 0.3mg given by me via auto injector IM right vastus lateralis and called 911 0954 BP 124/71, HR 84, RR 18, 99% RA 0956 50mg IM Benadryl right deltoid 1001 EMS arrived and patient transferred to ED

Approximately 10 minutes post vaccination, the patient expressed feeling "funny" and that his throat felt tight and he felt he was about to start sweating. His hands appeared red and were warm to touch, no other flushing noted. Blood pressure and pulse elevated. Benadryl 50mg administered PO. After 45 minutes of observation, vital signs returned to normal, patient noted resolution of tight throat feeling, feeling "funny" and redness of hands. D/C'd home with education regarding epipen use and emergency reasons to contact 911.

Member presented with profuse sweating and said he did not feel well at all. Because I do not have aassigned to the clinic, I went to ask an to do a physical examination on the member. The went retrieved water for the member to help him cool down and another patient was in need of my assistance. As I returned I saw the conducting his physical examination of the patient where he started to examine the member where the member did not present with any hives on his body but his tongue was slightly larger than normal and he started to clinch his stomach and he could not physically drink the water and he then fell over gasping for air and eyes bulging(you could hear him

He was ok right after administering vaccination. 10 minutes later he stood up and said he was feeling hot and collapsed on the floor. Seemed like he was having difficulty in breathing. Called 911 and administered chest compressions. He was partially alert during chest compressions and after 2 minutes of administering chest compressions he gained full consciousness and requested me to stop chest compressions and requested an ice pack to place on his forehead and also requested some electrolyte drink and they were provided. He sat up on the floor after a few minutes. He also said this fainting happens to him all the time when he gets any shot (injection). He was responsive and sat on a chair until paramedics arrived. on arrival they asked his couple of questions and checked his blood pressure and monitored his heat for a few minutes. He left with paramedics about 30 minutes from the time of vaccination.

Patient had trouble breathing (and is Asthmatic), and turned white and almost fainted, their brother went to the care to get their inhaler, and we provided orange juice and cold water for the patient.

Pt was feeling fine a half hour later and was called 4 hours later to follow up.

Patient vaccinated with Moderna COVID vaccine 3/25/21 and 4/22/21. Patient presented to Hospital 4/29 with SOB. The patient actually stated the SOB started 4/21 and he received his second Moderna vaccination 4/22. SOB progressively worsened from 4/21 until ED visit. Patient thought it was asthma flare. 4/29 04:00 CTA chest showing extensive bilateral pulmonary emboli, left greater than right. Started on heparin protocol for PE in the ER. 4/29 afternoon, patient is currently an inpatient at GSH on a heparin drip for anticoagulation.

Mother of patient reported he had shortness of breath in night after receiving the vaccine and was rushed to the hospital. Doctors at hospital prescribed him Proair and prednisone.

received his first dose of the vaccine on 01/13/21 and his second one on 02/10/21. Headaches started 24 hous after his second vaccine but stopped after 02/23/21. Aside from the headaches, nosebleeds also started and ended the same days. Patient went to the hospital for SOB on 02/21/2021 after awakening from a deep sleep to both a headache and nosebleed. The hospital kept him there overnight. NO further symptoms after.

Patient received second dose of medication after not concerns or issues with first dose. After approximately 25 minutes the patient started experiencing chills, dizziness and tightness in chest. The patient stated no issue with breathing but was provided water and was monitored with bedside care following concerns. Patient started experiencing labored breathing at approximately 11:50AM. Explained that his throat was feeling tight but was fine. Coached through breathing techniques and color was changing to more pale. Benadryl and Epi prepared. Patient was provided Benadryl as he was stating his throat did not feel "tight" rather he was feeling weight in his chest when breathing. 50mg of liquid Benadryl was administered at 11:52AM. Within 3 minutes patient color was returning and breathing was coming with more ease. Patient seemed more relaxed and stated he was feeling better. More water and close monitoring. Patient stated chills still present and fingers were cold 12:00PM. Provided blanket. Patient stated on sight from 11:00-1:00. Patient was feeling no concerns with breathing, chills were subsiding at 12:30pm. He was provided Benadryl, Tylenol and water to go home to father's care. He was provided option to be taken to walk in clinic or ER for further evaluation but stated that he felt comfortable with going home as he felt like himself.

Covid Vaccine was administered at 11:52 AM. At 11:57 AM patient began experiencing right arm numbness, loss of vision, experiencing hot flashes and shortness of breath. Vitals were taken : Temp:: 97.8 BP: 90/60 mmHg Pulse : 48 Spo2: 81 % Glucose: 89 Notified provider Fenny Lin, FNP Provided Nasal Ammonia inhalant and Oxygen was provided to patient at 2 Liter per minute for 20 Minutes. Stabilized patient Provided Gatorade for hydration Observed patient for an additional 30 Minutes per provider. After 30 Minutes of observation Vitals were taken at 12:37 PM Temp: 97.9 Spo2: 100% Pulse:51 to 57 BP: 92/62 mmHg. Symptoms resolved and patient was advised to follow up with his PCP for additional assistance. Also notified if symptoms returned must go to nearest emergency room for further evaluation. He may also contact our office for any further concerns. Per provider okay to release patient.

20 minutes after receiving the vaccine the patient began to describe feeling of malaise, fast heart beat and shallow breathing. Patient became visibly flushed and began sweating. Patient then complained of a bad headache. The patient's blood pressure was measured at 180/110. After lying down for a period of time the patient asked to have an ambulance called. The patient's father was called and informed of the incident at the patient's request. The patient then left with emt's when they arrived shortly after calling.

Three days post-vaccination #1, patient developed chest tightness and shortness of breath, has continued for past 3 weeks. Had syncopal episode on 5/26/21 and presented to Emergency Department at facility. Was found to be COVID positive with result on 5/26/21. Admitted to facility on 5/26/21, and diagnosed with pericarditis. Hospitalist suspects that pericarditis could be result of Moderna vaccine, as patient was previously young and healthy with no chronic conditions, with symptom onset 3 days post-vaccination. Patient also had expected post-vaccination symptoms, including flu-like symptoms, fever, chills.

Patient presented to ED via ambulance at 5:00am on 5/19/2021 after experiencing difficulty breathing and facial swelling about an hour prior. The patient has experienced significant allergic reaction in the past. On arrival of EMS the patient was noted to have difficulty breathing and angioedema of the lips. Patient received epinephrine 0.3mL IM and Benadryl 25 mg IV prior to arrival. On arrival to the ED the patient was noted to have no respiratory distress, signs of urticarial skin rash, vomiting, abdominal pain or diarrhea; no noted stridor or dysphonia; patient denied difficulty swallowing. ED notes state patient possibly ingested peanut-containing food prior to reaction onset, but this was not confirmed.

Patient c/o chest pain, shortness of breath, headaches, sweaty and shaky. EMS was called and patient refused to go on ambulance and left building with transportation.

sudden onset hour and a half prior to arrival chills, nausea, shortness of breath, sharp stabbing global chest pain with cough only nonproductive, tachycardia 120 bpm, subjective fever chills after having received moderna vaccine 3 days ago. Tachycardic to 120, active chest pain CTA negative

Dizziness, nausea, chills lasting 20-30 minutes tightness in chest / light pain near heart, lump/swelling in throat lasting 5-6 hours then tapering down for the next 4 hours

Patient received second dose of Moderna COVID 19 vaccine on 6/4/21. During this same time, patient also had URI diagnosed as bronchitis at an urgent care and provided albuterol inhaler. Cough and URI symptoms subsided on approximately 6/12/21 but developed SOB, lightheadedness, and palpitations thereafter prompting presentation to ED on 6/15/21. Patient was started on digoxin, heparin, and furosemide by cardiology team. Work up in progress.

patient c/o of "feeling of throat swelling", 25mg PO Benadryl given. symptoms persisted and patient given EpiPen 0.3mg autoinjector and transferred to ED via EMS.

Patient became dizzy/loss coordination and collapsed after completing 15 minute observation period. He was able to stand back up and walk to a chair with assistance from family members. At the immediate time of him sitting down after fall, he was unresponsive when asked his name. Approximately 5 minutes after initiation of episode he began to communicate and regain responsiveness but stated he continued to feel dizzy, sweaty, and had some difficulty breathing. At this time EMS arrived on the scene and took over the care of the patient.

Had a cough, fever and chills first night after vaccine. Continued to have flu like symptoms for 2 days. On the 3rd day after vaccine, woke up to chest tightness and pressure, shortness of breathe. "It Felt like an elephant sitting on my chest." Went to urgent care, abnormal ekg, went by ambulance to emergency dept. Troponins were elevated. Went to cath lab. Diagnosed with heart attack caused by vasospasm. Now on heart meds (plavix, metroprolol for 1 year) and low dose aspirin ongoing.
