The individual starts to show signs of anaphylaxis reaction some minutes after vaccination by the nurse. Said"I am not feeling good." Upon assessment, the individual is leaning his head backwards, unable to read how many fingers I held up; complained "can't see"; slow in responding. The individuals appears to be losing consciousness; eyes closed. Color of face pale. HR: 47; Pusle Ox 95%. RR:16. BP 112/64. Epipen 1 dose administered L leg IM. 911 was called. Continue to monitor and check vital signs. BP 143/75, HR: 54 and pulse ox: 94%. At 0850:EMS arrived. VS remain stable. HR: 62; Pulse Ox 94%. The individual starts to open up his eyes and responding. Parents at his side. He is taken to the ER. Followed up with the patient's parent, mom and is stable and doing better.

High fever of 102.5 that couldn't go down with ibuprofen or acetaminophen, achy joints, lethargic, loss of taste and smell.

pt presents w/ swelling bil eyes, no swelling noted to face, no other s/s, pt in no distress, eye swelling subsided w/in 30 mins. A&Ox4. mother with son, released home & encouraged to continue to monitor and report additional or adverse reactions to physician or to call 911 if difficulty breathing, pt will take benadryl po at home. mother states that this happened with 1st dose.

patient "saw black"and nearly passed out was grey, sweaty HR 39, BP 96/60 after a few minutes his color came back after vomiting x2 observed in tent, BP 136/78, HR 69

One hour after injection, felt his eyes swell and a tightening feeling in his throat. He went to the Emergency Room and was treated and released. They gave him oral Benadryl pills and observed him for approximately 2 hours. He stated that he felt better.

After getting vaccinated. Patient felt dizzy and shortness of breath, parking team sat him down in wheelchair. Medics responded, mouth feels dry. Feels a little light headed. HR 160, BP 130/85 at 9:30am. HR 151, BP 177/94 at 9:35am. Facial flushing. Loss of color in face. Transport to hospital.

Patient with generalized urticarial rash and eye swelling, no respiratory distress, no other systems involved besides skin, no mucous membrane involvement, taken to ER, treated with prednisone and benadryl, improved, sent home with benadryl, rash resolved in 48 hours

Red, swollen, warm to touch, and itchy at site of injection. About 1.5" X 3" area. One tablet of Benadryl taken. Red and itchiness went away. Still swollen and warm to touch. Also, eyes were dry and nose was runny.

18 y/o male reported having blurry vision approx.. 5 minutes after having his 1st dose vaccine. Pt walked to 15 waiting area prior to felling ill. Pt was siting down and had a syncopal episode lasting 20 seconds. Pt was Pale and cool to the touch. After he gained consciousness pt. stated he felt pins and needles to his legs arms hands and lower back. Pt felt better after gaining consciousness. 911 was called R22 arrived on the scene did an evaluation and obtained a pt. refusal as pt. did not want to go to the hospital.

Pt. just received first Moderna vaccine dosage. He had just sat down in the observation area when he began to feel flushed and lightheaded. NP was present and soon afterwards his head went backwards and he syncopized. He was caught by the NP and he was still sitting in the chair. NP proceeded to perform a sternal rub. Pt did not become conscious or aware for approximately 2-3 minutes. He never fell to the ground. On further questioning after patient was responsive, he states that he had not eaten prior to coming for his vaccination. He has only received a handful of vaccines and has never had the flu vaccine so he is unsure of his reaction. As per NP patients face became very pale PE: O2sat-96% on room air BP- 110/86 Pulse 60 Pt is pale in color Pupils slightly dilated Responds to sternal rub only After 2-3 minutes pt is more alert EMS was called but patient who is also here with his father refused to be transported to the ER. Repeat vitals 0pulse ox - 96 % room air BP- 105/65 HR-57 Pt. alert and able to respond to conversation Heart rate regular and easy Lungs CTA bilaterally Respiration regular and easy A/P 1-Gave patient water and 2 juices Symptoms have responded well to water and juice and sitting for a few minutes 2. EMS was called but patient refused to go to the ER 3. Discussed possible side effects of vaccine and when to call 911 or go to the closest ER 4. Patient left for home with father after being observed for 40 minutes

Blurry vision, light-headed. BPs 87/62 (pulse 81), 114/82 (pulse 68). Patient was dehydrated - advised to hydrate before second vaccine dose.

Patient received first dose of Moderna COVID-19 vaccine on 4/7/2021. After vaccine was given, patient complained of lightheadedness, numbness in hands, light sensitivity, and nausea. Doctors and an RN evaluated patient. BP 96/64, not tachycardic. Mother was driving vehicle. No meds given but patient sent with EMS to ED.

About 3 minutes after receiving the vaccine, patient complained of loss of vision, ringing in ears, lightheadedness, tingling in arms. Pt was diaphoretic and flushed. Pt was taken on a wheel chair to a gurney where they were assessed and vitals recorded. Patient was given some drinking water and symptoms resolved within minutes. Patient was observed for 20 minutes. Pt reported feeling better and fine and walked out of vaccine center on their own.

Pulsatile Tinnitus, verigo, dizziness, nausea, vomiting. When I searched the Internet, I found hundreds if not thousands of people reporting serious problems regarding their ears shortly after recieving a Covid-19 vacinne. Total or partial hearing loss, tennitis, vertigo, nausea,etc. This needs to be reported to the Public. I'm angry that I was not warned about this before getting the vacinne.

10 minutes following second dose administration, patient began to have blurred vision followed by syncope. After the syncopal episode, patient felt fine. Patient was provided some water to drink and is currently feeling fine.

It is thought to be vaccine-induced myocarditis. Myocardial infarction was ruled out. Patient does now have a reduced ejection fraction of 45%. Here is the note from the ED today: "He indicates that he was awakened from sleep with chest pain this morning around 2:50 AM. He ended up trying to sleep longer to see if it would improve and ended up going to his mother's room. She recommended that he drink some orange juice and gave him some Tylenol. He states that the pain continued to increase since that time and he asked her to bring him to the emergency department. The patient states that the pain is on the left side of his chest radiates to his left shoulder and down his left arm. He denies that it radiates to his back. He has had lightheadedness. He does not feel short of breath. He does have some pain into his left jaw as well. He has not had any vomiting. He denies abdominal pain. He denies diarrhea. The patient did receive his second dose of the Moderna vaccine on Friday to his left arm. He has had fevers over the past 24 hours to 103 at home. His mother and he indicate that the go away with Tylenol though seem to come back when the Tylenol wears off. He did complain of some headache, myalgias, malaise and sore throat which she states were all worse when he would have a fever and seemed to dissipate when his fever resolved. He states he did not and has never had chest pain like this until this morning. The patient does not have a history of Covid infection. He is active and has run track in the past. He has not had any travel. He denies any pain or swelling to his legs or calves. He has had no sick contacts. His mother received her second dose of the vaccine on the same day that he did and has not had any similar symptoms. "

Pt received 1st Moderna covid vaccine at approx 10:25am with no apparent issue. He was sitting outside pharmacy in observation area playing on cell phone and approximately 10 minutes after receiving vaccine fell in the floor, hitting his head. EMTs were called immediately as a precaution. He was conscious when pharmacist reached his side. He reported remembering a buzzing in his ears and then woke up in floor. Pt reported no hx of seizures or fainting. He remained in floor for several minutes, reporting his head hurt, but was able to tell his name, location, and date with no issue.

Pt developed fever 24hr after second dose. 2 days after second dose pt began experiencing chest pressure lasting 1 hour. Chest pressure reappeared at 03:00 6/14/21 with jaw pain. Ibuprofen made sxs worse.

Woke up on floor/ Past out; Anxiety; Fall; Little weak; Foggy head; Shiver; Headache; Nausea; Throw up; Concussion; Buzzing in ears; Did not eat; This spontaneous case was reported by a consumer and describes the occurrence of LOSS OF CONSCIOUSNESS (Woke up on floor/ Past out) in an 18-year-old male patient who received mRNA-1273 (Moderna COVID-19 Vaccine) (batch no. 033C21A) for COVID-19 vaccination. The occurrence of additional non-serious events is detailed below. No Medical History information was reported. On 01-Jun-2021, the patient received first dose of mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) 1 dosage form. On 01-Jun-2021, the patient experienced CONCUSSION (Concussion), TINNITUS (Buzzing in ears), EATING DISORDER (Did not eat), CHILLS (Shiver), HEADACHE (Headache), NAUSEA (Nausea) and VOMITING (Throw up). On 02-Jun-2021, the patient experienced ASTHENIA (Little weak) and FEELING ABNORMAL (Foggy head). On an unknown date, the patient experienced LOSS OF CONSCIOUSNESS (Woke up on floor/ Past out) (seriousness criterion medically significant), ANXIETY (Anxiety) and FALL (Fall). At the time of the report, LOSS OF CONSCIOUSNESS (Woke up on floor/ Past out), CONCUSSION (Concussion), TINNITUS (Buzzing in ears), EATING DISORDER (Did not eat), ASTHENIA (Little weak), FEELING ABNORMAL (Foggy head), ANXIETY (Anxiety), FALL (Fall), CHILLS (Shiver), HEADACHE (Headache), NAUSEA (Nausea) and VOMITING (Throw up) outcome was unknown. DIAGNOSTIC RESULTS (normal ranges are provided in parenthesis if available): On 01-Jun-2021, Blood glucose abnormal: abnormal (abnormal) abnormal. On 01-Jun-2021, Blood pressure measurement: abnormal (abnormal) abnormal. The action taken with mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) was unknown. Patient woke up with red spots/sores on tongue (likened to "burnt spots when you drink something too hot"), and they continue to persist. Patient has had trouble eating or drinking, as that increases the burning/tingling feeling.

Patient received Moderna COVID-19 vaccine on 6/22/21 at approximately 6pm. Returned to pharmacy on 6/28/21 to report the following. On Wednesday 6/23/21 at 5:30am patient has a fever. Then Thursday 12:00am patient reports tongue swelling, patient took 25mg of diphenhydramine and swelling subsided.

Right arm, R ear pain and dizziness, progressing facial numbness, possible focal seizures or syncope. No shortness of breath, difficulty swallowing or rash/hives. BP=129/92, HR=80.

Systemic: Dizziness / Lightheadness-Mild, Systemic: Flushed / Sweating-Mild, Systemic: Visual Changes/Disturbances-Mild

PT'S FACE LOSE COLOAR . HE WAS VERY WHITE. HE ALSO HAD COLD SWEAT. HE FELT DIZZY AND HAD RINGING IN HIS EARS HE FELT LIKE HE WANTED TO THROW UP. HIS AUNT WANTED US TO CALL AMULANCE. THE PARAMEDIC CAME . ALL THE VITALS WERE NORMAL. HIS FACE COLOR STARTED COMING BACK. PARAMEDIC ALSO CHECKED IF THERE WERE RASHES ON HIS CHEST AND ARMS. NOTHING. PARA MEDIC ASKED PT WHETHER HE WANTED TO GO HOSPITAL, HE DECLINED. HE SAT 10 MORE MINUTES FOR OBSERVATION AND HE FELT WELL ENOUGH AND LEFT.

Pt "blacked out" & legs went stiff shortly (within 5 mins of receiving vaccination), pt reported feeling cold & seeing black spots, emergency personnel was called immediately, water was given & monitored until emergency personnel arrived on site. Patient reported waves of feeling better & worse throughout the course of waiting for emergency personnel. Emergency personnel evaluated patient & transported to local hospital.

Jan 11-vaccination day. On Jan 14, in afternoon had tinnitus and muffled hearing that went away. The next morning Jan15, complete sudden hearing loss on left ear. Tinnitus and muffled hearing that has not went away.

I got the injection at 3:35pm on 1/20 in my left arm. Within the first 30 minutes, my shoulder around the injection site was incredibly sore, my left elbow had a lot of joint pain, and my left thumb and index fingers went numb within the first 30 minutes, I woke up at 8:00am on 1/21 and noticed that I had a light headache, felt extremely groggy, and was experiencing an uncomfortable amount of muscular and joint pain. I went to work at 9:00 and on the drive there, I began to feel dizzy and I felt as if when I turned my head, my vision would lag behind and take a second to process my surroundings. By 10:00, my muscular and joint pains were significantly amplified, my headaches were increasing in frequency and amplitude. I also began to have the chills, followed by periods of radiating heat from my body. Coworkers told me I looked very flushed, however, I never developed a fever. By 11:00, the muscular and joint pains, coupled with the headaches forced me to sit down and I almost broke down into tears. There were three sperate times where I began to get very lightheaded and thought I was going to pass out ( I did not). I went home at noon and noticed that my muscles felt like they were on fire. I took Tylenol and Advil whenever I could, which helped to reduce the symptoms. we had dinner at 7:00, however, I had no appetite, without eating since noon. On 1/22, I woke up at 8:30 with a pounding headache, muscular and joint pains still there, however reduced, and the same groggy feeling. This morning, I also noticed that I had a very sharp stomach pain in my abdomen. I also woke up with a severe nosebleed, which is extremely uncommon for me.

Immediately after the vaccine was administered I experienced the usual side effects of mild headache, fatigue and localized redness and swelling Around 11pm the night the vaccine was administered I developed a migraine that resulted in blurred vision. and when I checked my temperature it was approximately 104.9 degrees Fahrenheit.By the next morning, approximately 7am, I was weak to the point I could not get out of bed myself, I was nauseous that resulted in a single Emesis episode approximately 8am, I had mild swelling of my face since 7am, between 7:30am and 12pm, I had multiple times where I would temporarily lose my vision. Throughout 1/25 I continued to have a fever ranging from 102 degrees Fahrenheit to 103 degrees Fahrenheit. At approximately 8am and 1pm I took two acetaminophen pills each time which alleviated the migraine and to an extent helped with the visual deficit episodes. Throughout 1/24 and 1/25 I suffered with extreme chills resulting in shivering that lasted for several minutes despite a room temperature of approximately 90 degrees Fahrenheit and covered in two blankets. PCP was consulted via telephone and she advised to go to the hospital via EMS given the high grade fever, abnormal vitals and severity of symptoms

I called my insurance provider's RN hotline at the onset of dizziness, which was then followed by a bout of ear ringing and nausea. I was advised to seek immediate medical attention, so I had an RA at my university dorm walk with me across the street to M Health Fairview hospital. I was seen by an RN in their emergency room, but was ultimately never seen by a Doctor as I did not present a clear risk of anaphylaxis at the time. After waiting in the ER for four hours for medical care, I ultimately left because most of my symptoms subsided. My most lasting symptoms cleared up late yesterday (1/25) and they were extreme soreness/tenderness at injection site and fatigue.

Received COVID-19 vaccine (Moderna) on 1/21/21. No local reaction. No immediate symptoms (hives, lip swelling, respiratory symptoms). One day later, feverish T99.5F. 1/23/21, Patient developed a painful red rash on the palmar aspect of his hands and dorsum aspect of his feet and has had this rash since (total of 5 days). He currently takes Xyzal daily. Father would like to know if patient should receive the second dose of the vaccine. Erythematous macular rash limited to hands and feet that is blanching. As time progressed, raised in some places. Looked eczematous in some places as well. Painful, stinging skin, and swollen. He has limited range of motion. Hot shower decreased discomfort, increased range of motion. Cold water also helped. Symptoms are improving. Spring 2020 - may have had COVID - had anosmia. Was not tested at the time, but did have COVID antibody testing in July which was negative. Family has reported the reaction to VAERS and is also using V-Safe. Patient has no history of reactions to vaccines in the past nor has he had an allergic reaction to any component of the Moderna/Pfizer vaccines. He also does not have PEG/polysorbate allergy history (though he has taken Miralax in the past for constipation). Patient does have a history of anaphylaxis.

I received my first vaccine as the Pfizer manufacturer, and did not have any side effects. However, when I received my 2nd shot, I received the Modena manufacturer, and have experienced many concerning side effects. Fever, cold chills, trembling, fatigue, that lasted 2 days after the vaccine was administered. The side effect I am still experiencing is a metallic taste in my mouth, that gets worse when I drink water or other beverages. My hands and fingers turned a rust orange color the day after getting the shot and resolved within 48 hours.

Nose is stuffy; Trembling; Metallic taste in mouth; Hands and fingers turned a rust orange color; severe body ache; Headache; body chills; fever of 102F; Fatigue; Received first shot of Pfizer vaccine and scond shot of Moderna's; A regulatory authority report was received from a healthcare provider concerning a 19-year-old, male patient who received first shot of Pfizer vaccine and second shot of Moderna's/wrong product administered, headache, nose is stuffy/nasal congestion, chills, trembling/tremor, fever of 102F/pyrexia, hands and fingers turned a rust orange color/skin discoloration, metallic taste in mouth/dysgeusia, fatigue and severe body ache/myalgia. The patient's medical history included allergies to codeine and anxiety. Products known to have been used by the patient, within two weeks prior to the event, included Fluoxetine 40mg, Lamotrigine 100mg, Amlodipine 10mg and Lunesta 3mg. On 01 Feb 2021, approximately one day prior to the onset of the symptoms, the patient received a dose of the mRNA-1273 (Batch number: not provided) intramuscularly in the right arm for prophylaxis of COVID-19 infection. On 01 Feb 2021, the patient went to pharmacy and was offered his second dose of the vaccine, at the time of administration, the patient stated he was unaware that the vaccine given to him was from Moderna. His first vaccine was from Pfizer. On 02 Feb 2021, the patient stated he felt very sick, he had a headache, nose was stuffy, body chills, severe body aches, a fever of 102 degree Fahrenheit, and noted rust colored spots on his hands. The patient stated these symptoms lasted two days after the vaccine was administered. The patient also stated he had a metallic taste in his mouth that worsened when he drank water or other beverages. At the time of this report, the metallic taste in mouth had not resolved. The patient visited Urgent Care and was instructed to take a baby acetylsalicylic acid once a day. Action taken with mRNA-1273 in response to the events was not applicable. The outcome of the event, metallic taste in mouth/dysgeusia, was considered not resolved. The outcome of event, received first shot of Pfizer vaccine and second shot of Moderna's/wrong product administered, were considered resolved on 01 Feb 2021. The outcome of events, headache, nose is stuffy/nasal congestion, chills, trembling/tremor, fever of 102F/pyrexia, hands and fingers turned a rust orange color/skin discoloration, fatigue and severe body ache/myalgia, were considered recovered on 04 Feb 2021.; Reporter's Comments: Based on the current available information and temporal association between the use of the product and the start date of the events, a causal relationship cannot be excluded.

10 minutes post vaccine administration at 13:15 patient began to exhibit the following symptoms: dizziness, paleness, weakness, blurred/loss of vision, ringing in the ears, and diaphoresis. Cold pack was placed on patient's forehead and patient's feet were elevated. BP, pulse, & temperature unable to assess, pulse was faint and weak. Patient experienced loss consciousness for 1 minute, 34 seconds. During this time paramedics were called. Vitals at 13:20 were: BP=98/76, P=62, temp= unable to register with machine on hand. Patient able to follow commands and asked to lie down. Cold pack was placed on the back of his neck. Paramedics arrived and assessed patient's blood glucose = 56. Patient given orange juice. Vitals at 13:30 were: BP = 107/69, P = 70, Temp = 97.6., pulse ox: 99%. Patient asked to sit up and vitals were: BP = 114/73, P: 71. Patient declined to go be transported in ambulance with paramedics to the hospital at that time. Blood glucose assessed again = 127. Patient given chips and salsa from what facility had on hand. Patient stayed 10 minutes to eat the snack. Patient instructed to go to the hospital or urgent care for further follow-up and assessment. Patient verbalized understanding and agreed to instruction. Stated he will go to nearest Urgent Care facility. Patient left this facility alert and oriented.

A small amount of blood coming from the vaccination site caused the patient to feel dizzy and get tunnel vision. Patient may have lost consciousness but only for a second or two. Appeared diaphoretic, pale and unsteady. Was assisted to supine position and was responsive at that time with improved color, BP 128/82, HR 55, O2 98 within 5 minutes. Observed in supine for 30 minutes, re-evaluated vital signs at 15 minutes past laying supine with BP 122/76, HR 66, O2 98. Took orthostatic BP at 30 minutes with no drop in blood pressure.

Currently having it. I have a swollen face. Blurry vision. Difficulty standing. Shallow breaths. Headache. Chills. Body aches. 101 fever. Unable to sleep but very tired. Not able to breather through nose. Sore and numb right arm. Tingling of my limbs. Occasionally sharp pain in muscles. I got the shot at 11 and was just fine but I had work. I drove for a little over an hour and progressively my arm got weaker. I did physical labor until 10 pm and then started some school work, but found my arm almost unable to move and my mood was deteriorating. When I went to bed and lied down, it hit me. It is 4 am and it is still going strong.

Shortly after vaccine administration, pt c/o numbness to tongue, "difficulty" to back of throat, and shakiness. Dr. evaluates and finds no SOB, clear lung sounds, maintained 99-100% O2 sat on room air, and intermittent tachycardia. Benadryl and zyrtec administered, pt advised to eat as he reports he hasn't had much today. Dr. advises to go to ED if anything changes.

About 5 minutes after receiving the Moderna COVID vaccine, patient stood up to ask the pharmacy for a cup of water. His eyes rolled back and he gently fell to the ground. Opened his eyes almost immediately after, had a strong pulse and no shortness of breath. Patient states it was just nerves. 911 and store management was called just incase. The paramedics check patient out and released him. Patient's mom came to pick him up.

The patient became very pale and he could not see anything for few minutes. The patient felt better after 5-10 minutes

Patient complained of dizziness immediately after walking to checkout station. We sat him down and he said he was only seeing black and he was got and his neck hurt. We called EMS to check him out also. His BP was 140/90 and his vision started to come back after a couple of minutes as blurry. Once EMS arrived, he was much improved and vitals were all good. He refused to go to the ER, but his roommate assured us that he would watch him today. Patient waited 30 minutes and said he felt totally fine when he left.

This patient received Covid19 vaccine. This PT has a history of Autism and was getting anxious post vaccination. PT's mother was accompanying him when he began feeling dizzy, sweaty, hard of hearing and tightness in his Left arm where the vaccination was administered. PT was immediately evaluated by EMS and Fire Rescue on scene. PTs condition began to improve. PT declined transport.

Developed "moderna arm" (raised inflammation with itching 1.5-2 inches long and 0.5-1 inches across) 7 days post vaccination from injection 1. Developed somewhat severe tension headache on day 1 post vaccination from injection 2 (all further events listed below are from injection 2). Headache responded to ibuprofen. On day 4 post vaccination, began experiencing minor and infrequent migraine-like headaches that lasted for a few seconds to a minute. Moderate pain and localized. Pain site changed but was generally on left side. Experienced 0-2 a day until day 12. On day 11 post vaccination, began experiencing full, moderate tension headache. In addition, had more frequent migraine-like headaches. Did not respond to 400mg ibuprofen or 10mg Claritin. On day 12 post vaccination, began experiencing photophobia. Mentioned to general practitioner out of an abundance of caution who recommended Excedrin. Took two Excedrin extra strength (total dosage: Acetomenaphin 500mg, Aspirin 500 mg, Caffeine 130 mg). Tension headache ended and most (but not all) migraine-like pain subsided. Some occasional pain ranging from 1-2 out of 10. On day 13 out of an abundance of caution following reports of other events I went to the ER. The doctor there examined for any cognitive symptoms (asked me to follow her finger with my eyes, squeeze fingers, checked ability to move extremities, etc) and concluded no risk of CVST. Determined that vaccine likely lowered my threshold for headaches and migraines (I have had very infrequent cluster headaches lasting for a few seconds in the past, never for this duration). Final diagnosis was migraines. Doctor prescribed Fioricet and gave me a long-acting steroid. Some pain is still there, but symptoms have mostly stopped. Waves of quick bursts of pain on localized areas on the left side of my head are still occuring.

Within hours of getting the shot, patients left eye started turning red. He visited a medical facility and they diagnosed him pink eye and gave him antibiotic drops. The drops did not seem to be working and by that Monday he went to see the eye doctor. Eye doctor said it was viral pink eye and thought it was strange that this came about right after the vaccine and in the eye on the same side he was vaccinated. Over the next days and weeks, the viral infection moved to the other eye. Both eyes stayed swollen, red and tearing constantly. He has lost some of his vision and says that things far away are very fuzzy looking. The doctor said it was the worst case of viral pink eye he has ever seen and that he would have it for the long haul of 4 to 6 weeks. He had to do a medical withdrawal from college due to missing too much of the nursing program he is enrolled in. The pinkeye is still ongoing and we don't know when it will be over.

Patient had parked in the COVID vaccine waiting area, and was observed to fall forward onto his steering wheel within a few minutes of parking by 2 other participants waiting in the car next to him. One of them immediately notified one of the parking lot volunteers who came and got me within 1 minute of this observation. 2 students and I came quickly to the open drivers side door and began talking to assess the situation. Mr. Bannister appeared pale, mildly diaphoretic (sweating) and slightly confused as to the passage of time, but was able to state and spell his name and talk coherently about his circumstances. He stated that he was not allergic to anything that he knew about, and that he had no prior history of similar reactions to vaccines or medicines although he had not taken any shots in several years. He noted that he had not eaten anything since breakfast (now mid-afternoon). He described himself as initially feeling weak, faint, and nauseated with his vaccinated arm feeling heavy and numb. He also reported that he had a sensation of blurriness before passing out, but denied any spinning sensation, visual loss or ringing in the ears. There were no obviously visible abrasions or bleeding on his face or forehead. The nursing students brought a chair, and we elevated his legs and gave him some water to drink during the time we were monitoring his vital signs. He refused a cupcake because of the nausea, but as time progressed he gained facial color and was more comfortable in conversing. His hands had equal muscle grip and strength, visible fingernail beds were pink and his capillary refill was less than 3 seconds. Towards the end of our monitoring, he did accept some nuts and M & Ms to eat. A University Safety Medical Report was completed by the appropriate safety personnel. He was fully AAO x 3 at the end of our monitoring, and stated that if he felt better he would drive to his apartment (less than 5 minutes away) but would wait for his girlfriend to get out of class if he did not feel fully capable of driving. We encouraged him to eat something light when he got home, to use cool water/warm water soaks at his shot site, exercise his arm, and to take an NSAID for pain relief the next day. We recommended that he eat something light before his second shot (which had already been scheduled), that he bring someone with him the next time, and as a precaution to wait 30 minutes following the second shot. He left at 3: 15 pm driving his car without any apparent difficulties. An online report was completed for the incident 04/20/2021.

Approximately 2-3 mins after the injection, patient can to the pharmacy jittery, and stating he couldn't see and felt dizzy. Stated his pulse was felt low. Patient became pale and stated he has ringing in the ears.

pt was very nervous before the vaccine, he was okay right after he received shot and commented it was not bad. After vaccine he was sitting in post vaccine area he got up and said feels very light head. I came out of the pharmacy to direct him back to chair while holding him , he started fainting we sat him down. Eye was rolling and unconscious his mom kept calling out to him . His color was pale. We called 911. he was conscious mom gave him gatorade he vomited right away but said he is feeling better. I kept rubbing his back. He said he was nervous all day and had not eaten .

After the patient received his shot and instantly he began to sweat, felt thirsty, looked pale in color, unable to hear clearly and vision was blurred. He never completely fainted or fell on the ground . He felt no pain and his temperature was normal at 96.8F. After 10 minutes the patient symptoms started going away. When the paramedics arrived they cleared him to leave.

4/24/21 8:30 pm headache, dizziness, feverish, body aches 4/25/21 - 5/2/2021 all the time headache, dizziness 5/3/2021 -5:30 am headache and dizziness continued, it then at 6:15 am increased to point of almost blacking out, vision changes, then returned to previous state 5/4/2021 - 5/18/2021 headache and dizziness remained constant until noon when dizziness increased accompanied by hot flash, then returned to previous state

After having the shot, hedeveloped left arm pain and did develop some lymphadenopathy in his left axillary region. He was dealing with nausea and dizziness, and the following day, he went to work, although he was feeling a bit ill. While at work, he was found to have a fever, so he was sent home. He has been having hot and cold flashes since then. He has had a poor appetite. He has had a dry mouth and trouble sleeping. One day prior to admission, he then developed a chest pain that became constant and persisted, so he came into the emergency department on 05/19/2021 for further care. He has developed myocarditis after receiving the first of his Moderna Coronavirus Disease 19 vaccines.

He started with fever and chills on the evening of the day he received the vaccine, Also severe pain in lower sternum, emesis \over the next 2-3 days developed abd pain of lower abd, cough, resp distress, waves of fatigue and hyperventilation, on 5/10 noted full body rash which was itchy, last fever was on 5/13/21, on 5/12 had blood shot eyes, 5/14 - peely skin on palms and face and felt thightness in face

5 minutes after getting vaccinated patient stated having issues seeing "seeing black" I offered to call 911 but he did not want me to. I got him candy and water. 5 minutes later he vomited - he said he felt better but I asked him to stay for continued observation. I called him this morning and he said he just had a sore arm

The patient received his vaccine and 5 minutes later complained of feeling dizzy. He was instructed to sit and before we could help him into a chair he lost consciousness and fell. His fall was somewhat controlled by the technician. I then assessed the patient and determined he had a vasovagal episode and lifted his legs, he returned to consciousness within 10 seconds of raising his legs. I then allowed the patient to lie on the floor for approximately 20 minutes after which he felt better and returned to sitting . He then sat for about 15 more minutes until a family member was able to arrive. The patient reported no other adverse effects other than dizziness, lightheadedness, some trouble hearing and tingling fingers.

Patient started to feel dizzy and woozy approximately 30 minutes after the vaccination. Patient states he was having trouble with his vision and focusing on objects. States his feet were really itchy and he was sweating a bit.

Hives and rash all over the body, Angioedema like symptoms (significant swelling of the lips eyes, and face)

Within 24 hours severe headache, followed by inner war pain, then on Morning of 6/13/2021 awoke with left sided facial paralysis inability to close eye or lift smile.

PT WAS DISCHARGED HOME WITH FAMILY WITH INSTRUCTIONS TO INCREASE FLUID INTAKE. TAKE OTC ALLEGRA 180 MG EVERY DAY AND BENADRYL 25 MG EVERY 4-6 HOURS AS NEEDED FOR ITCHING OR RASH. NO DRIVING FOR 12 HOURS AND RETURN TO ED IF CONDITIONS WORSEN.

The occurrence of additional non-serious events is detailed below. No Medical History information was reported. On 05-Jun-2021, the patient received first dose of mRNA-1273 (Moderna COVID-19 Vaccine) (unknown route) 1 dosage form. On 05-Jun-2021, the patient experienced HYPERSENSITIVITY (severe allergic reaction/allergic reaction to the vaccine) (seriousness criteria medically significant and life threatening), PARAESTHESIA (fingers tingling), PARAESTHESIA ORAL (tongue felt abnormally large), URTICARIA (bumps on his face, back, chest and stomach), FEELING ABNORMAL (pattient felt very uncomfortable) and PAIN IN EXTREMITY (Sore arm). On 18-Jun-2021, the patient experienced NASAL CONGESTION (nose was congested), OROPHARYNGEAL DISCOMFORT (throat was scratchy), HEADACHE (really bad headache) and FATIGUE (feeling a lot of fatigue/feeling more tired than he usually is). Pt came to hospital 07/26/2021 for his first COVID-19 vaccine accompanied by his girlfriend. Pt completed appropriate paperwork. LPN reviewed paperwork with pt and gave pt Moderna vaccine Lot# 027B21A expire 09/18/2021 at about 1000 in Right Arm. Pt proceeded to the lobby with girlfriend. At about 1010 pt was witnessed falling from chair in lobby. Staff activated the emergency response protocol crash cart and 02 tank was brought to pt side within a few seconds. Staff called 911. On assessment pt was lying supine in lobby breathing normal and pulse WNL diaphoretic and pale. RN noted that pt eyes where rolling back. Pt head was supported, few seconds later pt came was awake. BP was checked 138/82 in left arm. Breathing WNL and HR WNL. Pt states "I was afraid this was going to happen I had this happen about 5 yrs ago with a booster shot". Pt girlfriend stated that he turned to her and stated I don't feel good and he leaned back into the chair then fell onto the floor bottom first then onto left side then onto back. Pt states he is not hurting anywhere other than where he got his shot. Pt was then assisted to the sitting position on floor. pt stated he felt better just week pt then was assisted to the chair. While sitting in chair pt was diaphoretic and pale pt was given a cold bottle of water and a cool wet paper towel. Pt continued to improve after that. EMS arrived at 1020. EMS got vitals and pt signed EMS document refusing transport to hospital. EMS left at 1027 Pt girlfriend called for a ride because he was driving. Pt back to normal by the time his ride got here at 1043 last vitals WNL BP 126/80.

Employee recieved the Maderna vaccination at 4pm 1/11/21. Employee was monitored for 30 minutes after vaccination and showed no side effects. He states that at 5pm he had a scratchy throat, at 8pm he had hives and his throat, chest and mouth started to itch. At that time he took Benadry and layed down. He woke up at 10pm with a swollen mouth and throat and was wheezy. He stated he had a hard time breathing and swollowing. At that time he took his EPI PEN. He did not seek medical treatment. Employee states that on 1/12/21 at 9:30am all symptoms had resolved.

2nd dose received at 11am. Mild arm soreness immediately after. Remained for 2 days with no increase in severity then dissipated. Lymphnode discomfort mild beginning 2 hours after vaccination. Remainder for 2 days with increase to medium severity approximately 8 hours after receiving vaccination. Nuchal rigidity began 8 hours after receiving 2nd dose, mild -> medium severity. Dissipated 1 day after receiving shot. Mild photosensitivity to light began approx 9 hours after receiving shot. Dissipated by next morning. 9 hours after receiving vaccination, medium fever developed. Baseline temperature is 96.4. At 9 hours after vaccination, developed fever at 99.7*F. Chills and bodyache following the #2 of moderna. Pt is tachycardic. Otherwise unlabored breathing. Steady gaits. 20 yo M w/ no known chronic medical conditions, p/w frontal HA, dry mouth, myalgias, fatigue, and nausea since last night. Received the COVID vaccine yesterday afternoon. No photophobia, vomiting, diarrhea, abd pain, cough/congestion, CP, SOB.

Started with a fever that slowly worsened to a high temperature. This resulted in chills, aches, pains, fatigue. nausea, congestion, sore throat, headaches, and weakness. There was also swelling at the injection site and swelling of my eyelids.

Client received 1st dose vaccine and remained in post vaccine monitoring, at the 15 minute point (end of monitoring) before leaving client reported feeling very dizzy with blurred vision. On assessment client was sitting up in chair, alert and oriented, V/S HR 72, RR 20, BP 124/72 O2 sat 98%. Lungs CTA, no reported difficulty with breathing. Client reported additional symptoms of itching, no rash noted and no change in skin color or appearance; nausea was also reported at this time. Information obtained from client on medical history, vaccine history, allergies and current medications. During this time period the itching and nausea was relieved, but the dizziness and blurred vision remained; V/S HR 74, RR 20, BP 128/78 O2 sat 98% . Client reported he started on a new medication Hydroxizine approximately 2 weeks ago and did experience dizziness in the first 2-3 days of starting the medication, but has not had that occur since that time. Client was unable to stand initially for BP check while standing, at 0945 additional medical assistance available to assist: sitting BP 130/76 HR70, standing BP 124/70 HR 70. Client reported no change but states dizziness and blurred vision continued and now he is seeing with blurred vision in what he reported at "cool and warm colors - like a filter on a phone". EMS was called at 0959, arrived at 1011 to evaluate and transport client to the hospital for evaluation. Client departed with EMS on a stretcher at 1014am.

Light-headed, fading vision, caused fainting post vaccine. Patient fell to the floor and had come back within seconds. Patient was evaluated in place for 1 minute and recovered. Patient was escourted to a chair and talked with a watcher for 15 minutes.

Pt. states that he became hot and dizzy while experiencing tunnel vision. A friend with the pt. stated that he fell out of the chair he was sitting in but did not hit his head on the floor. EMS and an on-scene Dr. made immediate pt. contact. It was determined that the pt. experienced a syncopal episode. He was already regaining consciousness upon arriving at his side. Vitals were taken and pt. recovered over time. Pt. signed an AMA and was taken home by a friend.

Patient reported stating feeling nauseous, lightheadedness, paleness, dizziness, and had trouble seeing to observation nurse, at 1116 AM. Vaccination nurse went to notify charge nurse on patient status. Vital signs included T 96.1, o2 99, RR 16, B/P 106/76, and HR 52 at 1116 AM. Supervisor on site notified the doctor via telephone at 1144 AM. Reassessed at 1126 AM second set of vital signs includes T 96.1, o2 98, RR 18, B/P 94/74, and HR 77.Currently patient does not display any s/s of distress or discomfort third set of vital signs includes T 97.0, o2 98, RR 18, B/P 116/86, and HR 78. As per MD patient is stable and can go home, clear to leave observation area. Patient instructed to contact HCP and/or go to ER if displays s/s.

After the patient received the immunization for Moderna, he sat in the waiting area for about 5 minutes and started to sweat profusely and have blurry vision. that eventually resolved after 15-20 minutes of rest, cold compress, and drinking water.

On 4/08/21 at 3:08 pm,20 years old Asian male received first dose of moderna Vaccine lot #026B21A , 0.5 ml to his let deltoid . At 3:24 pm Patient sitting in the observation area, he was complaining of blurred vision, feeling dizzy, diaphoretic, at 3:25 BP 120/80, pulse 72, 02 sat 98%. As per patient mother who currently in facility, four years ago after received his MMR and VRA vaccine: he had a seizure. Patient was placed on low dose of keppra for the seizure. However, patient has been off the keppra for five years as instructed by his neurologist. Pas medical history as followed: Seizure, HTN, Acne, Rhinitis, Sinus Brady Cardia. Patient currently on Acne mediation : Tretinion and Clindamycin cream. Po fluid given as per patient request, he stated ? I am feeling much better? 3:31pm rechecked Vital sign Bp 128/78, pulse 88, resp 18, o2 sat 97% on room air. Denies any further symptom at present. Patient observed for 30 minutes. Patient AAOx4, Ambulate independently, patient released accompanied his mother and father in stable condition.

symptoms fever, chills, and fatigue first occurred 4/9 shortly after receiving shot, and lasted until 4/13 on 4/10 vomiting and temporary blindness after exercise occurred. the blindness episode did not last longer than 2-3min, but was described as complete vision loss and occurred 1-2min after intercourse. on 4/14 an incident of ?blacking out? occurred during prolonged intense exercise

At 5:00pm. Blood pressure was elevated to 137/91, No temperature noted even though he felt hot and clammy. Gave him water, and put legs on a pillow and he lay down At 6:30 he began to feel sweaty, clammy and included nausea. These episodes continues until about 8:30pm when he started to feel a bit better After a night of sleep that was restless and about 6hr long woke to feeling better but tired.

During the 15 minute waiting period right after getting the vaccine, for 5 minutes I had experienced ringing in my ears, sweating, and complete loss of vision. These symptoms only lasted those 5 minutes and never came back.

Patient came to the pharmacy to get his first Moderna COVID vaccine today with his mom. After 15 minutes after receiving vaccine, he stated he felt dizziness, warmness, and flushing. I was able to elevate his feet and provide some cold water for him. After 10 minutes, he stated he felt his airway was getting closed and tongue was getting swollen. We decided to call 911 and provided Benadryl 50mg and Epipen for him. His blood pressure was 114/90 when EMS arrived. Since his vital was normal, EMS gave options to go to emergency room or urgent care. His mom and he chose to go to urgent care since he was getting better. I was able to follow up with his mom and he is going to take Benadryl for 2 days and follow up with Additional information for Item 18: his family physician. I will follow up with his doctor as well.

Arm soreness, tinnitus is much louder, dizziness, nausea, insomnia, fatigue, loss of appetite, diarrhea, On 4/27/21 I went to urgent care, they ran blood tests which showed possible signs of viral infection, was only prescribed zofran for the nausea and vomiting. Ears showed no outward sign of irritation or infection

Immediate malaise. Later in the day, eye puffiness. Next day with malaise and blurred and transient loss of vision (2-3 sec). Sweating and presyncopal without syncope. Later the second day feeling well, except mild malaise.

Pt reports that he felt lightheaded and thirsty. Stood up to go get a drink of water and in the process had syncopal episode. LOC lasted approx 5 seconds. Pt had not eaten breakfast prior to coming in. Pt was moved to a bed, hydrated, and monitored. Advised him to contact PCP.

12 minutes after injection - local itching/redness. 30 min. after vaccination anaphylaxis with multiple organ involvement - skin injection site and uvula swelling and eyes itchy. Patient had angioedema of throat with swallowing difficulty. Had mild asthma/wheeze. Also had tachycardia and light headed when trying to sit up. Patient given Epi 0.3 ml IM, Benadryl 50 mg IM, and Pepcid 20 mg PO. Transported to hospital by EMS for 4-6 hour observation. Patient responded well to epi.

Stood up to go to the observation chair after getting vaccine. After sitting down at observation area, heart rate seemed to drop/become abnormal, and I began to lose hearing, and vision began to get fuzzy. The pharmacist accompanied me as I was experiencing symptoms, talking me through things. I then began to get profuse cold sweats, after which I asked for water. After drinking a bottle of water and not moving for about 15 minutes, everything turned to relative normalcy. I was able to walk home afterwards.

Pt is an otherwise healthy 20 year old male who presents with 2 weeks of blurred vision in R eye. The blurry vision developed suddenly 1 week after getting 1st dose of covid vaccine (moderna). He was seen in ophthalmology clinic on 04/29 and diagnosed with R optic neuritis; The patient reports that his blurred vision has not improved or worsened since onset. It affects both his near and far vision in the right eye only. He denies double vision, eye pain, headaches, nausea, vomiting, numbness, tingling, or weakness. This has never happened before. No recent illness including fever, chills, runny nose, cough, or sore throat. Pt admitted to hospital. Etiology of his optic neuritis is unclear, but possibility include demyelinating etiology (like clinically isolated syndrome). Infectious etiology was thought to be unlikely given no signs/symptoms of infection and reassuring lab workup. No family history of similar symptoms reported, making hereditary optic neuritis less likely. Other considerations included other primary ophthalmological disease, although otherwise reassuring ophthalmological exam makes that less likely. Unclear association with COVID vaccination.

Patient became dizzy, nauseous, and had difficulty seeing approximately 15 minutes after his vaccine. Checked patients blood pressure which was low 70s/50s mmhg initially, patient remained conscious the entire time. He had no trouble breathing or swallowing. Patient was given a cold water and stated this helped. after initial BP reading EMS was called. Patients BP gradually rose and was 110/74 mmhg when EMS arrived. Glucose was also tested and was 101. Pt received his first dose of the Moderna Covid-19 vaccine. Immediately following the injection the pt began convulsing. Convulsions lasted for less than 30 seconds. Upon completion of convulsions pt was awake and alert. He stated he was thirsty. Before we could provide water to pt, the pt began vomiting. Pt was placed on his side and paramedics were called. Pt remained in a laying position until emt's arrived. Pt was examined and released. Pt was encouraged to follow up with his pcp.

Approximately 2-3 minutes after receiving the shot patient reports that he had a seizure. Was sitting in the observation area, felt like vision was blacking out. Observers report that patient stiffened in chair and eyes rolled back. No one observed tonic-clonic movements of limbs, but patient did have bladder incontinence. He was unresponsive for several minutes, EMS was called and evaluated patient at the scene. He started to feel better fairly quickly after regaining consciousness and refused transport to hospital.

Patient became pale, had chest pains, difficulty breathing, vomited, red spot formed on tip of tongue. 911 Was called. Patient was helped to the floor and a he was no longer vomiting. He was feeling better with chest pains relieved and no difficulty breathing. The red spot remained on his tongue and he was very pale. Paramedics came and he went to the hospital.

The patient said he is feeling nauseous and he said he cannot see after some seconds and also felt dizzy. we called 911 meanwhile the patient said after 5-10 minutes he can see and he still feel nauseous. He didn't go to the hospital with the paramedics. After paramedics left he said he felt fine.

Pt presented with fever, chills, severe abdominal pain, headache, photophobia and nausea a few hours after the second dose of the vaccine was administered. Few days later, pt was hospitalized comatose, and required intubation. Spinal tap showed lymphocytic pleocytosis but all viral and bacterial testing were normal. Patient started having trouble breathing about 2 minutes after his vaccination. Began to develop trouble hearing and having yellow vision. Within 5-8 minutes patient began sweating on his forehead and was feeling very tired. Patient was given water and 911 was called for more advanced observation. At the point fire fighters came to the seen, patient was feeling better and symptoms were almost completely gone.

the client receives allergy shots every 2 weeks. On 5-21-21 he received his first moderna vaccine with no problem. He received an allergy shot on 6-15-2021. On 6-16-2021, he experienced tingling on the left side of the tongue. His symptoms increased to being unable to smile on the left, unable to control blinking on the left. On 6-18-21 he went to Urgent Care and was diagnosed with Bell's Palsy and started on prednisone for 16 days. He reports that his symptoms subsided in 2 weeks before completing the prednisone.

about 10 minutes into his waiting period, I noticed he laid his head on his dad's shoulder. I went to check on him and he said he did not feel right. he described feeling dizzy and trouble seeing. I got him water, ice pack and took his blood pressure and used a pulse oximeter. His BP was 90/50 and hr was in the 50's o2 were perfect. we continued to monitor his and told him to drink more water. (he consumed 2 small bottles) before he left (30 minutes later) he was feeling much better. His dad said he was very nervous and has done this before with injections.

Just finished administering the Moderna vaccine and whilst still having a conversation with the patient I was about to start the clock on the timer to monitor the patient when his eyes rolled over, the patient slumped in his chair and fell to the floor. He jerked twice then his eyes opened and he looked very confused trying to figure out where he was. I let him stay on the floor for about two minutes and then I helped him get up and sit back into his seat. I monitored him for 30 minutes. There was no difficulty in breathing, shortness of breath or tightening of the airways. No swelling of the face or other sites of the body where observed. The patient was able to carry out a normal conversation. Once the 30 minutes where I released the patient and advised him to get in touch with his primary care physician, Dr.

Patient stated that he didn't feel well then his pupils dilated, his eyes rolled back in his head and he seized for approximately 15 seconds. Afterwards his pupils remained dilated and fixed until he regained consciousness. Patient was assisted to the floor where we waited for paramedics to arrive. The patient stated he was okay and refused to go with paramedics.

Itchy eyes. Resolved with 25 mg benadryl. Sore arm for three days. Chilly off and on for three days. Still some tired on third day. Does not know if this is related, but an old boil site started bleeding off and on and has not stopped.

Patient became lethargic, diaphoretic, complained of dizziness and eyes dilated bilaterally 15 minutes after injection was given. Patient was assisted to the floor. Legs were elevated. Vital signs were taken at 0635. BP 135/70, HR 75, Resp 16, O2 sat 98% on room air. Patient denied shortness of breath, throating swelling or pain. EMS was called at 0634. Epi Pen 0.3 mg was administered at 0638 in the left deltoid. Vital signs were taken again at 0642, BP 138/72, HR 106, Resp 20, Temp 99.3, O2 Sat 99% on room air. Patient verbalized that he was feeling better after epi was administered. Patient ate last at 0430. Interpreter was used to communicate with the patient. Interpreter contacted the patient's emergency contact. EMS arrived at 0644, patient was assessed and taken via ambulance. Epi pen was sent with EMS.

Lost vision for 2 minutes, felt lightheaded, faint. Those effects dimished after 5-10 minutes of deep breathing.

Immediately after injection I had soreness and several notable heart palpitations followed by mild dizziness and mild difficulty breathing accompanied with dry mouth. This subsided after a short while (within 15) minutes after I left the vaccine location. I arrived home and felt fatigue and a slight headache develop. The headache has since stopped. History: pt s/p 2nd Moderna COVID-19 vaccine yesterday and felt like having low-grade temp last night and felt like having hard time breathing deep with some itchy sensation in chest with deep breaths and woke up with fever and headache and muscle aches and took some Tylenol just PTA and feels significantly better currently. No rhinorrhea, cough, chest pain, vomit/diarrhea, rash, sick contact, recent COVID-19 hotspot visit, prior co-morbidities or surgeries.

Patient received Moderna #1 on 3/18/2021 at Hospital. The next day he developed a rash on his face and chest that was not itchy or painful. It lasted less than 24 hours. Then developed dizziness and vertigo for 3-4 days, episodes lasted a few seconds and noted that it happened more when looking at screens, had the sensation of spinning and a feeling like he may fall over. He has also complained of bilateral ear ringing and a dull pain in his left ear.

Patient received vaccine, then immediately described that he had trouble seeing and felt faint. EMS was immediately called. He had difficulty sitting up and kept his eyes closed. He had no reports of difficulty breathing. His body had started to get warmer within 5 minutes. Patient felt better around 10 minutes later when EMS arrived.

After receiving the vaccine while in the observation area patient's vision started getting blurry and he started feeling dizzy. (about 10 minutes after vaccine) He walked back to talk to us, while pharmacist was asking him questions he swayed and fainted. Pharmacist caught him and lowered him to the floor. Called 911 and paged MIC. Pharmacist stayed with the patient who gained back consciousness and placed him in recovery position until paramedics arrived.

Patient stated he started to feel unwell a few minutes after the vaccine, then started to feel light headed, nauseous, dizzy. He reported he felt like he was about to pass out, he felt he had dry mouth and was thirsty. To provider he appeared pale and sweating. He was instructed to sit in chair and lean forward with head down towards knees. He was provided water and his blood pressure was taken. The first reading was 89/66 pulse 74. He stated he was not sure what his normal pressure was, but was usually low. He also stated that he had a light breakfast, and didn't really drink much that day. After his pressure was taken he reported to feel slightly better, and was observed for another 25-30 minutes. After this time passed, his blood pressure was taken again, and reading was 102/74 and his color appeared better, and was more alert. After a few more minutes, he said he felt much better. He also had eaten crackers at this time. He was provided v-safe form again, and encouraged to sign up for follow-up, and to continue to monitor closely any symptoms for at least the next 6 hours.

Metallic taste in mouth that lasted approximately 3 hours; increased salivation after metallic taste began that went away after the metallic taste also went away.

21yo male reports with adverse effects of 1st dose Moderna covid-19 vaccine. Endorses decreased energy, sore arm, photophobia. VSS and WNL. Lungs CTAB. CV RRR no MGR. - reported in VAERS - tylenol 1000mg q8 hours - 24 hours quarters

Patient developed acute onset of hives on face and body and reported facial swelling and difficulty breathing approximately 8 days after receiving first Covid shot. Stated this occurred approx. 11:40pm 3/26/21. Patient arrived at ER 1:10am on 3/27/21 where facial swelling and rash on perioral area, upper back & proximal arms was confirmed. He was given prednisone 60mg, diphenhydramine 50mg & famotidine 20mg. Home RXs for diphenhydramine 50mg po q6h prn, famotidine 20mg po bid & prednisone 40mg po qd provided. He was discharged at 02:48. Patient now recovered but will not receive second dose of Covid vaccine. *NOTE: patient stated grandmother had similar reaction to Covid vaccination which occurred one week after shot.

Patient was sitting in the waiting room alert and responsive after his vaccine. Suddenly while he was sitting in the waiting room His eyes began to roll back into his head, his head dropped and his body tensed up and he began to shake. The event lasted approximately 30 and then the patient became alert and oriented again. When he became alert again he seemed very sweaty and pale. Nurse Practitioner took blood pressure and it was 100/58. Paramedics arrived on scene approximately 5 minutes after and transported him to the hospital.

My body and spescifically back hurt a lot and was somewhat impossible to focus 2-3 day from the vaccine

After receiving the vaccine, pt claims he can't see or hear well. Offered to call 911 or monitored until pt felt better. After 20 min pts vision was coming back and hearing was normal. 30-35 min later, vision and hearing came back to normal. Monitored pt for 1 hour and was okay to leave. verified if pt was okay and he was okay with no other symptoms. Temporarily experienced side effect. Pt will follow up with pcp doctor tomorrow just in case.

About 5 minutes after receiving the vaccine, he walked up to the in window stating that he felt his vision was diminishing. Then immediately he swayed and fainted. He came around quickly and paramedics were called. The paramedics assessed him and left without transporting him.

Approximately 5 minutes after dose was administered reported loss of vision for 3 minutes and then vision returned. He denied feeling light headed or dizzy. Blood pressure was 119/62 from left arm. No report of double vision.

About 5 minutes after giving the patient the shot, patient started feeling very dizzy, when asked if he would like an ambulance he refused. After a few more minutes, patient said he was feeling a little better. No trouble breathing or talking. After a few more minutes patient started holding his ear and said he was having trouble hearing and started seeing colors. We asked if patient would like an ambulance and patient agreed. Paramedics came and did some tests.

Patient experienced dizziness, fainting, sweatiness, and blurry vision 10 minutes after receiving the vaccine. Pharmacist asked patient to sit down and remove facial covering. Pharmacist took blood pressure, first reading was 104/56. Pharmacist offered patient water to drink and continued to monitor patient's symptoms for 40 minutes. Pharmacist took another blood pressure reading and the results were 129/73. Patient stated he no longer feel dizzy or feel fainting. Patient's father took patient home. Pharmacist called patient 2 hours after he left pharmacy. Patient stated he has severe headache. Pharmacist advised patient to take one Tylenol and eat some snack.

He reports that the same evening of the vaccine he noticed arm swelling, pain at the site and generalized body aches. He also reports that his tongue and throat felt numb for four hours. He also noticed a little cough, shortness of breath and a runny/stuffy nose.

Blurred vision, difficulty breathing (pale skin/blue lips), profuse sweating, muscle fatigue, headache. This lasted about 15 minutes. Until severity went down. Followed by 20 minutes of profuse sweating and headache. I thought I was going to die

Pt received first dose of Moderna Covid Vaccine in left arm; approximately 1345 Patient was sitting in waiting area when pt verbalized that he was experiencing swelling of the tongue and having difficulty breathing. Patient could be observed with profuse sweating. Epi Pen x1 0.3 mg IM was administered patient's right thigh, Emergency department immediately notified by medical staff and ER physician and Nurse arrive in approximately 3 minutes to assess patient. Pt transferred to ER for further care and observation.

STARTED WITH BODY ACHES ON DATE OF VACCINATION WITH POSSIBLE TEMP. ON FRIDAY (2 DAYS POST) PT. WOKE UP WITH JAW PAIN AND SHORTNESS OF BREATH.

Day of 2nd Moderna vaccine Zizzo developed sore throat, loss of taste/smell, headache, myalgia, fatigue, fever/chills.

"Moderna COVID-19 Vaccine " pt received vaccine to his left arm. 10 min later (1335) he reported feeling dizzy. allergy tech and i evaluated pt. pt clammy with sweat noted to the back of his neck. pt speaking in full complete sentences. no increased WOB. left radial pulse regular and not elevated, not thready. no hives found. denied itchiness. asked pt if he felt anything stuck in his throat, he swallowed a few times and said "yes". asked if he had much water today, he said no. pt reported it felt a little hard to breathe in his throat. i had him remove his mask. pt reported nausea. denied abdominal pain. had him take a few sips of water. reported he no longer felt anything stuck in his throat, no longer hard to breathe in his throat. pt reported tingling in his left arm. pt worked the night shift. went to work at 1600 on 2/11. ate at 2000, beef and potatoes. had snacks. ate chicken and rice at 0300. usually has a full gallon of water on shift but only had a half gallon. got off at 0400. went to sleep at 0530. usually wakes at 1445-1500 but woke up earlier for this appt. appt was at 1330 but he was here early. had a protein shake 30 min before arrival but has not had much water. thinking the protein shake was causing his symptoms. denies working out today, denied energy drink intake, denied any coffee. pt took a large amount of water and then threw up. pt reported he felt much better. nausea resolved. still no itchiness. no hives. no clamminess. no trouble breathing. no globus. no dizziness. no tingling in his left arm. resolved by 1340. held Epi given he reported complete resolution of his symptoms. seemed likely the combination of being underhydrated, having slept less than usual, facility being a little warm and him wearing a mask and possibly his protein shake contributed. had patient wait 30 minutes (1410). allergy tech sat with patient who continued to speak in full sentences, tell stories, etc. had him stand up slowly. denied dizziness. still no hives, no itchiness, no globus, no trouble breathing, no nausea, no abdominal pain. advised pt of reasons to call 911 (he lives on base where we have ambulance services). advised i would be placing a VAERS report.

After about 15 minutes the subject asked to use a pulse oximeter, reading normal. Then said it was like looking in a tunnel. We asked him to sit an d applied an ice pack to the back of the neck. At the time this provider did not know the patient had a seizure disorder. He slid to the floor with full tonic clonic involvement. His head and neck were protected. 911 was called and an ambulance. After about 3 minutes the subject regained consciousness. He did not wish to be checked out at the hospital. He signed a release and his mother picked him up

Patient got lightheaded and clammy. Eyes rolled to back of head about 3 minutes after injection. MT assessed pulse, slow but steady. RE got BP cuff and took BP. 1st BP reading was 146/96 with pulse of 59. Sat O2 was 100% Staff went to get patient water per patient request. Resident claims he 'lost hearing' for a small time period. BP reading at 1147 was 148/91. SS, CNP, was summoned to vaccination area to assess patient. Spoke with patient / family. He has had similar reaction before after knee surgery. Patient feeling better. CNP requests continued observation until DBP is below 80. BP reading at 1153 is 135/79. Patient released from observation and left vaccination clinic.

0936 vs 92/60, 58, 20, 98%. Pt c/o ringing in both ears, tightness in head and skins were pale and diaphoretic. Pt denies any SOB, rash, hives, dizziness. Pt?s father is here with him and is his driver. 0946 vs 120/82, 69, 20, 97%. Pt is feeling a bit better. The ringing in the ears and tightness to head is gone, BP has increased and skins are drying and color is returning. This is his first vaccine. Pt did not want any further medical attention/paramedics called. Pt and father were advised to call 911 if any further symptoms occur after leaving site. Pt is feeling back to normal with skins W/D, no other symptoms.

PT DISCOVERED INATTENTIVE AN SQUINTING HIS EYES. ONCE ALERT, PT C/O OF LIGHT HEADEDNESS, SWEATING, BLURRED VISION, PARESTHESIAS IN BOTH ARMS AND LEGS. VS @ 3:08 BP 78/62 HR 86 REPEAT VS @ 3:20 PT GIVEN GATORADE FOR REHYDRATION AND INSTRUCTED TO BREATH SLOW LY AND DEEPLY. AFTER 10 MINUTES, PT SYMPTOMS STARTED TO RESOLVE. SYMPTOMS COMPLETELY RESOLVED WITH TIME. PT BECAME STABLE TO LEAVE OBSERVATION SITE. INSTRUCTED TO F/U WITH PCP.

Temporary loss of taste a day and a half after receiving the first dose, fever, extreme fatigue, congestion. Tested for Covid-19 on the first day of symptoms and came back negative. Day 5 after the dose and still having congestion. Fever only lasted for one day, on the 27th.

Tinnitus developed two days after vaccine. No evidence of Tinnitus before vaccine. Ringing has not stopped in both ears since March 13.

Sluggishness, profuse sweating, feeling overheated and dehydrated, in/out of alertness, and "not being able to see" Patient reported symptoms within 5 to 10 minutes after receiving vaccination. Symptoms resolved over 15 minutes.

Pt. did not eat or drink, feels numbness in face and tongue. Body felt heavy (only had a protein milkshake this morning) BP: 130/26; HR: 65

I started to salivate a uncontrollably for hours on April 14, at around 1am. Still happens now sporadically. I had nerve damage to my tongue from oral surgery so I dont know if its related. I started to develop vertigo today the 15th around 1pm. Vertigo has come and gone on waves. Have had a mild headache all day as well. In addition Ive had alot of trouble concentrating.

Symptoms: Extreme pain immediately after injection at site, closely followed by racing heart and winded and dizzy, followed by sweating, blurred vision and confusion. Don't remember hitting horn to alert officials. Got out of car, dizzy and stumbled. Blacked out and fell to pavement. Don't remember falling, woke up with extreme mind cloudiness, drowsiness, and hands were numb to touch but felt extreme pressure on palms. When pressure subsided, finger tips felt inflamed.

Pt started feeling dizzy, eyes rolled, lost consciousness for a few seconds, lethargic, nauseous, pale, low BP

* Tuesday, March 16th, I received my first Moderna shot. No immediate symptoms, felt fine. * Thursday, March 25th (9 days post-vaccination), I was sitting at my desk and experienced some kind of visual distortion (my computer monitor bounced up and down 4 times in quick succession, even though I was perfectly still). Got up from my desk and laid down, and it went away. * Tuesday, March 30th (two weeks post-vaccination), I started experiencing persistent dizziness/imbalance. It was manageable for the first couple of days, and I still went about my daily activities. * Thursday, April 1st, symptoms got noticeably worse. I had a really bad episode of vertigo/dizziness in the afternoon. This spell of bad symptoms lasted until Saturday evening/Sunday morning. I wasn?t falling down, but I would describe it as akin to being on a boat in rough seas or a plane flying through a storm. Pressure in head and ears was intense. I?ve had longstanding tinnitus, but pitch/volume definitely seemed to change somewhat. My neck was incredibly stiff, and my throat felt somewhat ?tight? (tonsils or lymph nodes in neck felt enlarged). Took ibuprofen and Benadryl on and off over next several days. * Tuesday, April 27th, symptoms are ongoing. Still having imbalance, dizziness, and non-spinning vertigo, though less severe than previously.

Patient was given their first dose of the Moderna COVIDvaccine. Within 1 minute, the patient began having a seizure (jerking, eyes rolled back in the head, head went limp, and his body stiffed up), and then went unconscious.

Pt reports becoming warm, clammy, with minor tinnitus in left ear. No hx of allergy. Placed in semifowler position on cot. BP 110/80 HR 60, RR 14 SPO2 98%. at 10:05 SPO2 up to 99% and he reports being asymptomatic. He stood up with no vagal response.

Sense of taste completely destroyed. Meat tastes like sulfur or metal. Rancid. It?s lasted for at least 5 days so far. Can?t eat meat because of it. Can?t taste the majority of vegetables. The only things that taste good/normal are sweets, because of course Aching pain in right calf/hamstring but not in left

On Day 4: Swollen lips; canker-like sore on lower lip, upper gum; Day 5: Swollen lips, Swollen tongue with increase in canker-like lesions.

Fever of 101.5 F following day of vaccination. Lethargic, tired. Severe loss of hearing in right ear described on May 17, 2021.

I received my second dose of my moderna COVID-19 vaccine on Sunday, May 9th. On May 13th around 2:40 pm, I began to experience a full sensation in my right ear. My hearing felt slightly muffled, with mild ringing. It has been 5 days since my symptoms started. On May 17th I went to my primary care provider and was told I had redness and inflammation inside my ear. I was prescribed sterile otic suspension drops of Ciprofloxacin 0.3 % and Dexamethasone 0.1% for use of 7 days. This is my second day of treatment, however I feel the same.

After getting the first round of the Vaccine, I had the loss of smell and taste happen as if I had covid again, and it's lasted for a month now. It's affecting my appetite.

Patient got up from seat to get water, begin to see lights, fell forward/ employee caught him, didn't quite remember falling, we had patient lie on ground, elevate feet, placed cold pack on head, and cloth underneath head. Called EMS, they arrived shortly

ABOUT 30 SECONDS AFTER THE SHOT WAS ADMINISTERED THE PATIENT HAD A SEIZURE WHILE SITTING IN THE CHAIR IN THE WAITING ROOM. FULL BODY TWITCHING AND EYES ROLLING FOR 15 SECONDES OR SO. LEFT ARM WAS ALSO SWOLLEN AT THE ELBOW WHICH WAS OPPOSITE ARM OF THE IMMUNIZATION. PARAMEDICS WERE CALLED AND PATIENT WAS EVALUATED IN THE AMBULANCE BUT CHOSE NOT TO GO TO THE HOSPITAL AND TOLD THE PARAMEDICS HE WOULD GO ON HIS OWN. STILL HAS NOT BEEN SEEN PER PATIENT.

At the end of April my left foot begun having arthrisit like pain. My foot was and still is tingly and I experience sharp shooting pains in my foot. After this began I was diagnosed with Bells Paulsy on May 3rd after I noticed on May 1st the left side of my face was weak and I could not blink my left eye.

1st Shot(5/3/21): Soreness in arm, Fatique 2nd shot(5/31/21): Soreness in arm, Fatique, headaches, developed a cough a week later which lasted about a week. Adverse event (7/6/21): Started coughing and spitting blood around 5:00pm. Coughing persisted throughout the evening. At 6:00am I told my mom I was having a problem. around 6:30am my body went numb, loud ringing in my ears, and broke out into a cold sweat, so we called 911. The paramedics arrived and checked me out, and they believed I was okay. When they left we decided to go to the ER. Admitted on 7/7/21-7/12/21

7/6/2021: Hemoptysis, ringing in ears, felt like fainting, shortness of breath -- Right Lower Chest Pain -- EMS called 7/7/2021: ER evaluation showed Acute Pulmonary Embolism --- > Admitted to hospital--- > IV Heparin --- > Xarelto. Also IV Rocephin + Azithromycin. Blood Cultures Negative x 2. EKG Sinus tach 103. Light Hemoptysis continued x 3 days. Venous Duplex Bilateral Lower Extremities Negative for DVT. 7/10/2021: Bilateral Upper Extremity Venous Duplex Negative. 7/12/2021: Discharged Home

patient received moderna vaccine at 12:15. at approx. 12:20 patient stated that he "felt weird" patient appeared pale, diaphoretic, and complained of losing hearing and blurred vision. we sat the patient down and took his vital signs and called 911 at 12:20. Patients blood pressure was 88/42, heartrate was 55 BPM, SPo2: 99%. Patient never lost consciousness and was talking to us throughout the entire event. Retook vital signs at 12:22 BP: 88/55 HR: 53 Retook vitals at 12:25 to BP :94/54 HR: 57 O2: 99%. At 12:30 Ems arrived, and assessed patient, patients mother was called at 12:25 to notify of event, patient declined going to the hospital. Patient stated he would wait for mother to arrive and leave with her. Mother arrived and drove patient home at 1:00 pm Patient stated before leaving HD that he didn't eat any food today but he felt a lot better.

Patient received second Moderna vaccine at vaccine event. He had reported some post vaccine tingling with first vaccine, so he was triaged after second vaccine to the 30 minute observation area. He again complained of left sided tingling and also subjective numbness of face and tongue. His exam was normal. Normal cranial nerve function. Normal speech. Normal VS. No meds administered. Given some snacks. Symptoms resolved over course of another 30 minutes. Discharged to home without incident

Approximately 2 minutes after receiving vaccine, patient exhibited a numbness in the tongue and deeper areas of the mouth. BP 134/80 HR 78. Slight numbness has remained fairly constant at 11:55am. Patient reported symptoms subsiding (or less worrisome). Patient clinically stable. Patient released at 12:10pm.

Early on in the day of 1/6/21, I felt very sluggish and spaced. My boss asked me early in the day if I was okay because she said that I seemed "out of it." It wasn't to the point that I was unable to work, but it was a noticeable difference from my typical state. At 11:31 AM, I texted my wife and said "I feel gross and weird today." When she asked me how so, I replied, "I just feel run down, my neck hurts, and I can't focus." My arm was more sore than it typically is from the flu shot, but it wasn't unbearable. As the day went on, I felt the cloudiness in my head start to dissipate a bit and I started to feel a little better. By dinnertime, I was feeling sick to my stomach and was not able to eat very much. I chose to head to bed around 9 because I was feeling pretty ill (my neck had really started feeling sore again and my stomach was very upset.) At 1:30 AM on 1/8/21, I woke up with a horrible headache. I get migraines from time to time, but this was unlike anything I had ever felt before. It felt like my head was in a vise and I was very close to asking my wife to take me to the ER. I never go to the doctor, but it legitimately scared me. My neck was extremely sore and my head hurt so bad that even laying it on a pillow hurt. I was extremely nauseous as well. I took a few extra strength Tylenol and laid my neck on a heating pad with an ice back on my forehead. The sound of our humidifier was too much for me, so I also out in earplugs. After an hour or so, I fell back asleep. When I woke up this morning, I still had a headache and I took an Excedrin. To this point, I have felt a little sick to my stomach, but the intense headache has been kept at bay. My neck is still sore but it is also somewhat better.

pt c/o of flu like symptoms after receiving 2nd COVID vaccine dose at 1100 02FEB2021. Pt reports fever (tmax 101F), chills, N/V, generalized body aches, h/a, sore throat, and dizziness. Pt states flu-like symptoms after getting 2nd COVID vaccine shot yesterday. Pt states fever, chills, n/v, dizziness, HA, sore throat, bodyache. Pt's left tonsil swollen. Pt states 1 episode of vomiting today 0100 without blood, 8/10 constant bodyache. Pt denies taking medication PTA.

Moderna COVID-19 VACCINE Fever, chills, cough, sweating overnight and then loss of taste and smell the next morning. Soreness at the site of the injection. Extremely fatigued. Tmax 100.5 Bodyaches, Headaches , Nasal congestion, Shortness of Breath, Chest Pain. Four days after 2nd Moderna vaccination his loss of taste and smell is still affected. His left arm soreness still persists. Bodyaches and fatigue still persists. Headaches comes and go. Nasal congestion persists Resolved Shortness of breath,Chest Pain, Fever and cough

Fever at 7am at 2/20. Fever is 102.2. Extreme fatigue, sore muscles. Hard to walk and keep balance. Dry mouth. Head ache. Numbness in arms

Nothing at time of injection. In the evening, soreness in the arm (strong but on par for what I would expect). Next morning, I starting feeling sick, with flu symptoms. Aching muscles, sensitive skin, mild headache, sore eyes, and in the evening, nausea. Temperature gradually rose. ~99.7 at 6pm, 100.1 at 7pm, 100.7 at 8pm, 101.7 at 9:30pm. I took some Tylenol around 7pm, and then some theraflu at 10pm. Fell asleep at 11pm. Periodically waking up every 30min from 1am onwards. Muscle discomfort, sweating, and nausea continued until 3:45 AM, at which point I was feeling better and was able to sleep continuously. Feeling much better in the morning on March 4th. Temperature was 99.9, then rose to 100.0 at 11am. Now, at 3pm, feeling fatigue, exhaustion. No headache. Normal sense of taste and smell. No couch. Taking another day off from work. Temperature is currently 100.0. Taking more theraflu now, have not taken Tylenol today.

Patient received a COVID-19 vaccine on 04MAR21 @ 1400. At 1645, he returned to the vaccine room shaking and his eyes were tearing up. He complained of pain at the injection site, headache, nausea, abdominal pain, chills and joint pain. His temperature was 98.4 @ 1712. He also complained of increased saliva production. He laid down on a cot and an emergency rescue blanket was applied. He was reassured and monitored by our onsite RN. He was stable and left at 1755.

Became diaphoretic, pale, clammy, faint B/P 111/71, HR= 50 Stated feeling dizzy, ringing in ears, unable to remain alert/focused. LOC intermittently for several seconds for about 10 minutes total time

First vaccine on 1/26/21 began taking three naps a day and arm pain. Given Ibuprofen. Seemed to resolve after 5 days, but continued to nap three times a day sometimes. No fever, but hot flashes at times. Booster given 2/23/21. Began to nap again three times a day or want to stay in his dark room. Began to have a lot of sinus drainage. Gagging at times. Pain over right eye area. Pain in right flank and stomach area. Severe headaches and possible body aches. No fever, but increased and more severe hot flashes that would wake him. 2/28 had a tonic clonic seizure after presenting with a distressed look throughout the day. Slept and upon waking began vomiting until 3am. Was at the ER and given many tests, pain meds and nausea meds. Ruled out kidney stones with CT scan and urinalysis. No fever, but hot flashing continued. ER visit lasted from 2/28/21 through 3/1/21 ER Physician suspects possible reaction to vaccine. Followed up with Neurologist and General Practitioner and both suspect reaction to vaccine.

Lightheaded, blurring of vision . Paleness , diaphoresis Hypotensive and bradycardia. Shock . Remained awake BP 58/30 Pulse 40 / min Immediately sent to Emergency Department

Patient was waiting for a 15 minute observational period. During the observation he started experiencing dizziness, light headedness and loss of hearing. Patient stood up to notify observation staff of his symptoms and had a syncopal episode and was moved to a cot. Patient soon regained consciousness and observational staff took a first set of vital signs with BP 128/72, O2 97% and strong pulse of 108 Bpm. Patient continued having dizziness for approximately 10 minutes after the event occurred. Onsite medical staff offered an ambulance transport to a nearby medical facility and patient reused against medical advice. Medical staff took a second set of vital signs with BP 124/76, O2 99%, strong pulse 78 BPM. Medical staff took patient out on a wheel chair and patient had a relative drive him home.

The patient experienced body aches and headache within the first 24-48 hours of receiving the vaccine. On 4/1/2021, the patient had swelling around his eyes at around noon. He stated that he feels that the swelling worsened that night but this morning (4/2/21) the swelling has improved. He came to the clinic this morning to check in regarding his symptoms. I called in PA to assess the patient. She assessed the patient and he stated that he had some SOB yesterday but denied any SOB today. The patient's O2 sat was 99% and HR was 98 bpm. She recommended that the patient take Claritin BID and return for follow up on 4/5/21 to see her in clinic. She advised him to go to the ER if he experiences any SOB, increased swelling, or difficulty breathing and he verbalized understanding. The patient has a hx of HTN and takes Lisinopril 10 mg daily. He did take his medication this morning. The patient denied taking any new medications. He also denied any previous vaccine, drug or food allergies.

Patient (002956225) - received Moderna vaccination (Lot # 0011A21A) on 04/16/2021 at 09:02 in the left arm by RN. 0904 Patient began walking from the Vaccine station towards Observation. After approximately 20? feet, LPN in another vaccine station down the line, noticed the patient stumbling towards the wall. She observed him stumble/fall into the wall, hitting his head, then began falling to the floor. LPN came around her partition, called for help and was immediately assisted by LPN. The two nurses assisted the patient to the floor. They reported his eyes were fully dilated and he began having light, full-body shakes. The two nurses turned the patient on his side and held his head. Waters said the patient was able to answer all questions appropriately. The full body shakes lasted approximately 30 seconds. The Patient did not lose consciousness. 0905 Lead Nurse arrived noting the patient was lying on the floor in the prone position exhibiting mild seizure like activity and his pupils were dilated. Patient was rolled onto his side until the seizure like activity subsided, lasting for approximately 30 seconds. 0905 IC/PSC called 911, PD. The site Medical Protocols plan was activated. 0906 Vital signs were obtained: Blood pressure: 96/80, HR 78, 02 at 91% on room air. A neuro check was performed on the patient and he was noted to be alert and oriented x3. Patient stated he had not eaten anything prior to coming in. Patient was assisted to a seated position while still on the floor. Patient was given water, juice, a banana and other snacks. Nurses in the Observation Station noticed the incident from across the arena and brought the EpiPens, AED, Blood Pressure Cuff and med bag. 0910 Vital signs were obtained: BP 104/47, HR 50, 02 98% on room air and respirations 18. Patient remained fully alert and oriented but noted to have mild tremors of both hands. 0915 TFD, followed shortly by EMS, arrived on scene at and assumed care of the Patient. 0926 Patient decided to be transport to the Hospital and was removed from facility via stretcher.

Starting about 10 hours after receiving the second Modern vaccination I experienced a minor headache and drowsiness, which continued until about 30 hours after the vaccination. These symptoms resolved without treatment. Around 20 hours after the vaccination an itchy rash developed on the lower lip, around the left eye, and in the genital area (including: penis, testicles, and upper inner thigh). Rash continued to get worse until about 4 days after the vaccination. At this point I sought treatment at a local urgent care. The urgent care personnel said they believed the rash symptoms were caused by the Modern COVID-19 vaccination, did not identify any other possible causes, and encouraged me to report it to the VAERS system. The treatment followed was topical 1% hydrocortisone cream applied to the affected areas, and 12.5 mg of diphenhydramine taken orally before bed. The rash symptoms improved with treatment but have not fully resolved at 5 days after the vaccination.

Swollen eye lids (top and bottom), rash on face, hands, necks. Itchy all over. Throat tenderness and tight feeling

Patient received immunization and went to sit-down in post-immunization area. Their eyes rolled back and they started shaking. The episode lasted less than 1 minute. After this, patient was alert and oriented. Upon speaking with the patient they stated that they have this same reaction every time they give blood. They did not disclose this during the patient question or the pre-immunization interview with RPh. Patient felt hot and clammy, so we got patient an ice pack and advised they stay seated. We took patient's blood pressure and heart rate, both were normal. After monitoring patient a while longer patient stated they felt okay to leave.

Extreme fatigue, pain in the arm, then swelling in the area around eyes, red-eye, and swollen red eyelids, then the onset of rapid heart rate, difficulty breathing, extreme exhaustion, and low energy first onset of massive grand mal seizure occurred approximately 24 hours after injection.

patient passed out 15 minutes after receiving shot. fell out of chair onto floor. helped up and sat in chair. pt was pale, tunnel vision and could not answer questions. gave some water and a granola bar because patient had not eaten since night before. called squad, pt refused treatment. stayed for another 30 minutes until determined ok to leave on his own volition.

Systemic: Dizziness / Lightheadness-Mild, Systemic: Flushed / Sweating-Mild, Systemic: Tingling (specify: facial area, extemities)-Mild, Systemic: Visual Changes/Disturbances-Mild, Systemic: Weakness-Mild, Additional Details: 5 minutes after vaccine was given, patient complained of blurry vision, lightheadedness, dizziness and L arm tingling and weakness extending to the digits. Resolved within 10 minutes after oral fluids and feet elevation. No lingering effects >2 hours after this event.

Ichyness on tonsils and back of nasal passage similar to inhaling vapors from spicy food. Popped blood vessel in left eye small pink area around bottom of retina. Eye still has vision

The evening of vaccine, patient started experiencing ringing in both ears and loss of hearing. The next day, hearing and ringing had not improved. Patient began vomiting and experienced extreme vertigo. On the third day, condition had not changed. Patient's urine is dark and he cannot keep down any food.

Patient reported sweating and blurred vision shortly after vaccine admin. Gave patient water . Observed patient. Called 911- They observed patient-patient was fine-no sweating. vision ok- EMT said patient had a Vagal response- nervous reaction to getting a vaccine. Patient reported having a similar reaction when younger and getting a vaccine.

high fevers (104-105) several weeks after vaccine with negative infectious work-up. Also with anterior cervical lymphadenopathy and cracked lips. I am concerned for Kawasaki disease flare-up although did not have all typical features of this disease

No immediate reaction to the shot on 5/15/21. 2 weeks later 5/29/21 noticed music suddenly sounded different. Audiogram 6/1/21 confirmed 10-20dB loss in left ear on most frequencies. ENT diagnosed with Sudden Sensorineural Hearing Loss and started on prednisone. 6/2/21 Audiogram showed 5-15dB improvement in most frequencies <8khz but 30-50dB loss in 12.5-16khz range. No tinnitus reported. No change in hearing or tinnitus since then.

Pt became lethargic, slow to respond, speech muttered, shaking, drooping right eye, nauseous. BP 169/108, O2 sat=98%, pulse 100. Paramedics called and patient transferred to hospital.

A few hours after the injection, I started feeling severe joint and back pain. Around 5 PM is when I started feeling a tingling feeling in my hands, feet, and around the outside of my mouth. I still have a loss of feeling in my feet after returning to work today, 1/25/2021, and I was promptly sent home after informing my management. I still have some loss of feeling around the corners of my mouth, and most of the feeling has recovered in my hands.

On 2/24/21 around 1:30 PM, Patient was not feeling well and temperature was 100.8. He was able to take Tylenol but then the staff noted his eyes rolled back in his head. 911 was called but he was not transported since his temperature was now normal and he seemed to be at baseline. He did not eat or drink much liquids later that night and in the morning of 2/25/21 around 9 AM, staff could not get him up from bed. He was incontinent and had temperature of 97.7. He was staring off and not talking or responsive. 911 was called and he was transferred to the ER and later admitted. All labs and Xrays were normal and later on 2/26/21 he was awake and alert. Of note, Patient also received news on the afternoon he got his second vaccine that a favorite staff person had been killed in a MVA. Patient was upset at this news at the time.

Patient received a COVID-19 vaccine on 05MAR21 @ 1600 at our onsite clinic. He became light-headed, he was seeing "spots" and had cold sweats. He laid down on a cot. His blood pressure was 126/82, heart rate 64, respirations 14 and oxygen saturation 95% at 1615. He slowly sat up, drank some water and was monitored an additional 30 minutes by our onsite nurse. He was instructed to follow-up with our onsite nurse with any additional concerns.

Loss of hearing in both ears; Fatigue; Joint pain; Headache; A spontaneous report was received from a Healthcare Professional concerning a 24 years old male patient who received Moderna COVID-19 vaccine (mRNA-1273) and experienced fatigue, joint pain, headache and loss of hearing in both ears. The patient's medical history was not provided. Products known to have been used by the patient were Tylenol and Ibuprofen. On 26 Jan 2021, prior to the onset of the events the patient received their first of two planned doses of mRNA-1273 (lot/batch: 041L20A) intramuscularly in the left arm for prophylaxis of COVID-19 infection. On 27 Jan 2021, the patient experienced the events fatigue, joint pain and headache. On 28 Jan 2021, the patient experienced the loss of hearing in both ears. The patient was treated with Tylenol and Ibuprofen. No relevant laboratory details were included. Action taken with mRNA-1273 in response to the events was unknown. The outcome of events include fatigue, joint pain and headache were unknown. The outcome of the event loss of hearing in both ears was not recovered.; Reporter's Comments: Very limited information regarding this event/s has been provided at this time. Further information has been requested.

Pt experienced fainting sensation shortly after receiving vaccine. Pt was sweating and reported temporary loss of vision after getting up after the vaccine. Pt informed pharmacist of 2 previous incidents of vasovagal syncope and was instructed to receive the vaccine while sitting on a cot. Pt was instructed to lie down flat on the cot for 30 minutes to be observed by pharmacists. Blood pressure and pulse monitored (normal range 126/66mmHg and 76BPM). Pt fully recovered after 5 minutes. Pt was instructed to seek emergency help if any new symptoms develop or worsen.

PATIENT WAS VACCINATED WITH SECOND DOSE OF MODERNA COVID VACCINE AND THAT NIGHT EXPERIENCED ITCHING OF THE EYES, FEVER, AND BODY ACHES. NEXT MORNING PATIENT WOKE UP WITH RASH AROUND EYES. PATIENT SEEKING CARE AT CVS MINUTE CLINIC.

Severe rash on toes, feet, legs, mouth, hands, and upper arms. Swelling in feet and toes. Injection site has become very hot to touch. Rashes began on 04/03/2021 on just the back of heels but then spread quickly to top of feet and toes within just a few hours. Used diphenhydramine gel and cream on feet on the night of 04/03 to be able to sleep. Rash on injection site and on the same region of opposite arm intensified on 04/04/2021. Intense itchiness called for more ointment on both upper arms injection site still hot to touch and ring rash has become more visible. Foot and toe rash returned at 8 pm 04/04/2021 and is unrelenting. Bumps appearing on toes.

The patient became dizzy and felt as if he might pass out after 15 minutes. He then was instructed to lay down and elevate his legs. After 15 more minutes he felt fine and was able to walk out of the store. Later in the evening he called the pharmacy and told us his arm and tongue were tingling. He was instructed to seek medical treatment if the symptoms became worse. We spoke to him later and he stated that his symptoms went away without the use of medications.

Per RN, "Patient here with worker group to receive 1st moderna COVID vaccination. Patient received vaccination approx 1436. At 1446 patient had complaint of dry lips and statement that he has allergy to something but unable to recall. Pulled patient to recliner chair and using interpreter asked about symptoms further. He then stated that his throat was feeling like it was tightening up and swelling. At 1448 administered emergency epi pen dose to right outer thigh. Patient was then rolled into building and presented to the emergency department for evaluation. Within couple minutes of the epi shot, he stated that he was starting to feel better. That statement was as we were rolling down the hall towards the ED." Patient taken to the ER to e seen from 1450-1620. RN in the ED, notes throat and mouth dryness while in the ED. Patient was monitored without complication and discharged home with employer representative.

Nose running, taste of bile, tiredness, started Monday 4/5/2021 around 5pm EST, persisted overnight Still present at time of report

Patient reports that within 2-3 minutes (10:06 AM) after receiving the vaccine that he felt dizzy and light headed with blurry vision. PT pulse was 50 with BP 90/60. EMS called and evaluated patient. BP was 88/52 when EMS arrived and pulse was 60. Before EMS left BP was 100/74. Patient reported feeling better prior to EMS arriving but was still evaluated by EMS.

I received dose #1 of COVID and planned to walk through the store to shop. While walking passed the observation chair, I began to feel dizzy and decided to sit down. After I took a seat, I immediately began to feel numbness in my arm which spread to my entire body. I was also feeling a piercing headache with ringing in my ears and my dizziness was extreme. At this point I called for help to the individual who had administered my shot as I had feared I would have fainted. I was in this extreme state for approximately 3-5 minutes. The individual at the pharmacy then checked in on me, gave me a bottle of water and after drinking it I began to feel better. I was then able to walk around and buy some food. Approximately 20 minutes after that event and eating and drinking some food, I felt like I was back to normal. I am writing this 5 hours later, and I currently feel normal, with some soreness in my arm I received the vaccine in.

Two weeks after vaccine Headache for six days Followed by tinnitus Tinnitus ongoing for ten days and has not stopped

Lightheaded 18hrs of the second vaccine, developed over the next 4 hours into a bad headache, minor nausea, light sensitivity. I think it may have been a fever but I couldn?t find a thermometer. I just remember feeling really hot.

Patient after the vaccination during the waiting period started having vision changes, dizziness, shortness of breath, itching, paleness, sweating, throat a little tight. 911 was called, and patient monitored very close with Epipen out and ready to use if needed. 911 team arrived to store patient had some water and was starting to feel better. Patient ended up feeling fine and it was thought after all tests to be an anxiety attack. No medication was given. Patient did not go to ER.

Pt became dizzy and light headed shortly after receiving his Moderna shot today. He sat down, then dizziness became more intense and vision was becoming affected. Immediately gave him some water, symptoms were worsening so we moved him to the immunization room so he could lay down. Started fanning paper to cool him down and got ice packs. He started feeling better about 5 minutes later. He also received some almonds, chips and water, as he stated he did not eat breakfast and did work an overnight. He left without incident about 45 minutes later, felt fine and back to normal. He did mention he gets lightheaded previously giving blood.

Pt was sitting in post-vaccination area for his 15 minute observation time when he passed out after a few minutes. He had fallen onto the floor beside his chair. I immediately went out to pt. His eyes were fluttering. I shook pt and asked if he was okay. Pt opened his eyes and sat up seemed disoriented and said that he felt nauseated. I told pt to lay down on his back and elevate his feet on the chair, and told my technician to grab a bucket, an ice pack, and to call code white. We applied the ice pack to pts head. A member of management and an EMT who happened to be in the store reported over very shortly. Pt said he did think that he hit his head and his head did hurt. We asked the pt if he wanted us to call an ambulance. Pt said no, he was feeling a little better and he just wanted to wait a little bit and then finish his shopping. Pt was feeling better and was able to get back into the chair and sit. Pt appeared pale and we got him a drink of water. Pt sat and rested for sometime and after about an hour he said he was okay and was going to finish his shopping.

Sudden partial loss of hearing in right ear, followed by tinnitus starting 4 days after the vaccination. Symptoms only present in the right ear, and there is no history of tinnitus or ear problems.

Pt report feeling clammy and "vision going like about to faint about 2 to 3 minutes after the shot. Pharmacist went over to check and found that patient is sweating. Patient also complained about nausea, hot and thirsty. Pharmacist did not find any rash or swelling on the face. Patient drank some water and did some slow, deep breathing. He reported feeling better after about 10 to 15 minutes. Pharmacy staffs called 911 when pharmacist first checking on the patient. EMR arrived in about 10 to 15 minutes and got blood pressure 125/76 heart rate 60 to 64. Patient said that he feels okay and EMR that he does not need to go to hospital. Patient agreed to check with MD and let pharmacy follow up next day.

Patient was seated completing the15 minute observation period and at 1349 was witnessed by clinic staff dropping his phone, leaning over to pick it up and falling down onto the floor. As nurse approached, patient laying on his right side below the chair with full body convulsions lasting approximately 15 seconds. Patient's head supported and protected, eyes rolled back, skin appeared pale, incontinent of urine. Patient appeared to regain consciousness for a few seconds and then began to convulse again for 5 seconds. Patient then regained full consciousness with no postictal confusion.

Between 5:05-5:07 Patient reported blurred vision. Went out to examine the patient and patient was diaphoretic with enlarged pupils and pale. He then stated that he could not see and stopped responding to voice and touch. After approximately 1 minute patient regained consciousness and said his vision was starting to return. Approximately 5 minutes after he said his vision had returned to normal, his pupils had returned to normal size.

Approximately 5 minutes after receiving the vaccine, the patient stated that his right eye was swollen shut. The patient denied itching, shortness of breath. The right eye was not edematous and no erythema noted. The eye just appeared to be squinted shut. The patient's vitals were monitored for 30 minutes and he was given Benadryl 25mg by mouth at 10:20 am. At 10:40, his eye was completely opened. The patient denied any shortness of breath, itching, or difficulty swallowing. He was dismissed from the clinic alert, oriented, and ambulatory without difficulty.

Patient's mom called at pharmacy on 5/17/21 to report patient experienced side effects like muscle aches, headache, 101.8 fever at night on 5/15/21, spine and teeth ache and injection site was elevated and hot to touch. Mom said patient felt better after taking Tylenol and most side effects have subsided, but she just wanted to report about side effects. Patient feels much better today (5/17/21) so mom said they are not planning to make doctor visit.

Tinnitus 18 hours after second dose. Loud high pitched ringing that gets louder during quiet, when trying to sleep. Sometimes too loud to sleep. Never suffered from tinnitus in my life, musician very careful about hearing protection. Haven't seen doctor yet but I will if it doesn't go away. Reading a lot of other people have dealt with this for weeks and months after vaccination.

Patient experienced nausea, dizziness, light headedness, sweating, ringing in the ears. I gave the patient an ice pack and he put it on his neck. Adverse events all resolved within 5 minutes of the ice pack.

Severe flu-like symptoms. Woke up at 4am with a splitting headache, my ears ringing loudly, and my whole body aching like crazy. Severe dizziness any time I stood up. Slightly short of breath. High fever and chills. I?ve been taking ibuprofen every few hours and drinking lots of water, and I have felt better as time has gone on. Right now I?m still slightly achy but I don?t have a fever anymore and the dizziness and chills are gone. My appetite is starting to come back too.

PATIENT WASN'T FEELING WELL AFTER VACCINE (WAS A LITTLE NERVOUS) AND ASKED FOR SOME WATER. PATIENT SAT DOWN IN WAITING AREA WHILE I WENT TO GET HIM A DRINK. WHEN I CAME BACK PATIENT WAS RIGID AND LOCK JAWED AND NOT BREATHING. WHEN WE GOT PATIENT TO FLOOR HE WAS RESPONSIVE TO A STERNUM RUB AND HAD A PULSE. AT THIS TIME HE STARTED TO PRODUCE SNORE LIKE NOISES, CONVULSED FOR LESS THAN A MINUTE THEN CAME BACK TO. EMT'S CAME AND CLEARED PATIENT. PATIENT STATED HE HAS A VASOVAGAL REFLEX HOWEVER HAS NOT HAD AN INCIDENT IN MANY YEARS. IT IS UNKNOWN IF THIS IS WHAT PATIENT EXPERIENCED OR A REACTION TO THE VACCINE.

Pt reports that 15 days after vaccination, he developed a erythematous, pruritic, hive-like rash over bilateral legs and left arm that cleared up after 24 hours without intervention. The following day (16 days post-vaccianation) he noticed a petechial rash over bilateral anterior thighs with no known injury to thighs and small amount of bleeding when brushing teeth.

PATIENT DESCRIBED HANDS FEELING LOCKED IN POSITIONS, EYE SENSITIVITY DESCRIBED AS BRIGHTNESS AND FEELING WARM. IT HAPPENED A FEW MINUTES AFTER SITTING ON CHAIR. HE SAID HE FELT BETTER AND WENT AWAY ON ITS OWN BUT THOUGHT HE SHOULD LET THE PHARMACIST KNOW. HE SAID HAD NOT EATEN YET EITHER. HE DID NOT WANT TO WAIT ANY LONGER AT PHARMACY UPON RECOMMENDATION AND WOULD LET PHARMACY KNOW IF ANYTHING ELSE HAPPENS AND/OR WILL GET IMMEDIATE CARE IF NECESSARY.

Patient stated could (taste) vaccine. I had patient wait awhile, but stated he was leaving after about 15 minutes. I counseled patient that if he was uncomfortable, or has any other side effects, to go to emergency room.

Patient was not vaccinated at our site, so I do not have information regarding lot number, etc. Vaccinated with dose #2 of Moderna vaccine 10 days prior to admission. Patient began feeling poorly, with fatigue and weakness the day following his vaccination. 10 days post vaccination, mother found him with right arm weakness, facial droop, and tongue deviation. Patient was severely encephalopathic with moaning, eyes deviated, unable to follow commands, diaphoretic. No focal deficits noted. CT head negative for acute finding. Platelet count low at 24,000. LDH elevated at 2856. UDS negative. Patient suspected to have aTTP and treated appropriately with high dose steroids, plasmapheresis, and rituximab.

Numb in hand, pain in arm that radiated to jaw and across the chest. Having trouble speaking, balance issues, extremely confused most of the time. Symptoms of heart attack or stroke.

One month after second dose, mild tinnitus (had for seven years previous) became severe and began causing ear pain/headaches.

Patient started feeling dizzy, hot, and nauseated then his vision started to "black out." He vomited and his heart rate was elevated. He laid down on the floor for a few minutes. Then he sat down for about 20 minutes and he said he felt a lot better.

Patient received his first dose of the Moderna Covid vaccine and was advised to wait for 15 minutes after the vaccine for monitoring. Immediately after the vaccine, patient reported feeling light headed and blurred vision. He also stated afterwards that he had previously fainted after a blood draw. A certified pharmacy technician administered the vaccine and immediately notified the pharmacist once patient reported symptoms. Patient was given a bottle of water and instructed to remain seated and take deep breaths. He refused offer to call an ambulance. After about 5-7 minutes, patient reported feeling nauseous and vomited. Immediately after vomiting he reported feeling much better. He was instructed to remain seated and stay for observation for another 15 minutes. Patient remained stable and stated he was okay to leave. Followed up with patient in an hour and he reported feeling just fine.

Approximately 12 hours after injection, I experienced hot flashes, chills, and a severe headache. The next morning I experienced a less severe headache but experienced noticeable shortness of breath especially on exertion. Tylenol P.O. helped my headache that morning, no known fever. Approx 2 days after injection I also experienced a noticeable reduction in the ability to smell or taste. This Stayed for at least another day or so.

Pt states "I woke up, ate breakfast and around 8:30am I started feeling dizzy and having blurred vision. I was working at the time and went to my local EMS station to get checked out. They checked my blood pressure and blood sugar. Blood pressure was normal but my blood sugar was low, around 68. They gave me a coke to drink and I went back to the Sheriff's office and about an hour later my blood sugar was around 90. I took the rest of the day off and come home. I took Extra Strength Tylenol and rested. I did have a headache a few hours later. I feel fine now but I have a mild headache."

After receipt of the vaccine, I developed a low grade fever the next day (1/7/21) that persisted throughout the day until it progressed into a fever of at least 101.3 F at night. It might have been higher, but that was the best reading I could get. It felt like the fever came on extremely fast bc all of a sudden I started shaking vigorously with rigors and chills. I couldn't get out of bed without experiencing the most uncomfortable chills I've ever experienced. I previously had Covid in early November, and this fever felt worse than what I experienced when I actually had Covid. Also, I experienced moderate to severe pain in my left arm on 1/7/21 at the injection site, which was far worse than any other muscle soreness I've experienced from any other vaccine I've received. The hour preceding the sudden fever attack, my left arm was feeling blazing hot, looked a little red, and swollen. I also experienced a strange allodynia type pain, whereby parts of my body hurt from simply touching them. Although I did not develop a significant rash, I experienced pruritus all over my body, predominantly affecting the areas around my ankles and wrists. I have a hx of multiple dermal filler injections in my lips. I don't think I had any problems with my lips after the injection. But other symptoms that I developed included a tickle in my throat and some shortness of air. On 1/8/21, I woke up drenched in sweat so I presumed that I broke my fever while sleeping. I have felt much better ever since. I'm a nurse practitioner student, and I had to miss a day of clinical on 1/8/21 bc of this reaction to the vaccine. I used ibuprofen the morning on 1/7/21 and Tylenol at night on 1/7/21 to bring down my fever

Became pale and lightheaded. Vision faded to almost completely black, but was remedied by lying down. After lying down I heard a static/white noise sound in my ears that progressively got louder. It hit a peak volume then slowly started to get quieter until it went away. Muscle soreness in injected arm.

About 5 minutes after the vaccine, I felt my vision going dark and blurry. I then lost consciousness and was told my eyes rolled back and slightly convulsed. I regained consciousness after a short time frame.

Patient received his 2nd dose of the Moderna vaccine prior to hospital admission at unknown site. Patient reported generalized body aches and that his whole body hurts and feels likes he's "being poked with a knife." He endorsed associated symptoms of fever, nausea, back pain, and shortness of breath. The patient denies any sore throat, headache, cough, or vomiting. His back pain is at the tip of his shoulders, in the middle of his spine, and near his kidneys. The patient adds the abdominal pain is worst in the lower left quadrant. Patient admitted for bilateral lower extremity weakness limiting him to ambulate and to rule out Guillain-Barré. During admission, had fevers, photophobia, phonophobia, and nuchal rigidity. On physical examination patient was able to minimally wiggle toes, had no plantar/dorsiflexion, unable to move hip joints or knee joints bilaterally, lift his legs against gravity.

Patient awoke the morning following the vaccination experiencing double vision, difficulty focusing vision. He presented to opthalmologist on 1/30/21 and was diagnosed with a L 6th nerve palsy. His symptoms have persisted and are accompanied by a HA.

Patient was feeling dizzy and had a low blood pressure, looked very pale, he claimed to have blurry vision. After sitting for a while he started to feel better and blood pressure rose to his normal value. Patient stated that this type of reaction to a vaccine had happened before.

"had covid shot, then developed aching in his L side of face and aching in his L ear. This started 4d ago. no fever. He is using otc drops. not helping."

loss of smell occurring 3-4 days after first vaccination. loss of smell lasting more than 5 days and fully recovered within one week

The morning after I received both vaccine shots, I posted hearing from my right ear. The first time I received the vaccine shot, the morning after, I could barely hear out of my right ear. It almost sounded like there was a clog or earwax build-up. I didn't really think much of it and it gradually went away several days later. The worst day was the next day after the vaccine and it lasted, even though in a small amount, until 3 days after. The second dose caused the same problem. The morning after the second dose of the vaccine, I lost my hearing in the same way as before. I could barely hear out of my ear, however, I was also exhausted and ended up sleeping most of the day. When I woke up in the afternoon, I couldn't hear from my right ear, but I could hear better. (maybe 25% back to normal). The two days after the vaccine, my hearing came back way better than the previous time. It felt like 80% back to normal and later in the today, it felt like it up to 95% back to normal. The best way I can describe the feeling was as if the sound was still being received to my hear, but it sounded muffled and turned down. I was still able to hear the deeper sounds during conversations and deeper 'beats' in music, but the sharpness of everything was turned off.

Moderna COVID-19 Vaccine EUA The day after I received the 2nd dose of the vaccine I had a headache. That same night I ran a fever and woke up the next morning feeling ok. I was taking a shower and it was reported to me by my fiancé I had passed out, started to seize, my eyes rolled to the back of my head. A few minutes later I woke up on the floor and asked what had happened. I was not sure what had happened. I felt fine when I woke up. When first responders arrived they asked me if I knew who I was, the date, the president, where I lived to all I was able to answer correctly. I went to the ER and they did a ECG, blood test, urine test, chest x-ray, CT of my head and they said everything came back normal. I was prescribed Keppra and have an MRI scheduled in the coming week.

Pt was in seated position in a chair and slumped over to right side and fell to floor. Pt appeared to twitch slowly for aobut 20 seconds. Pt was brought to seated position on floor. Appeared pale and diaphoretic. Pt verbalized he did not realize he fainted and loss consciousness for about 20-50 seconds. BP: 67/27. HR: 36. MDP doctor called over to assess situation. 2nd set of vitals taken. BP 51/27. HR 41. Pt given water. States he feels dizzy and light headed. Vision is "weird." Sees colors more vibrantly. The Dr. recommends pt go to ED. Pt transported via wheelchair by the Dr and clinic NP with Anaphylaxis kit. In ED, pt's BP was 86/60. Blood Sugar 91. Pt admitted for fluid hydration and monitoring.

Had a sore left arm where the injection site was, after about an hour later i began to have a mild headache and became tired (i took 2 aleve liquid gel pain killers and that didn't do much). The next day the pain in my arm got about 50% better, and the head ache was gone right when I woke up. I still felt tired throughout the 2nd day and my left under eye lid began to start twitching for about a minute long every 30 minutes. On the 3rd day, I was no longer tired and my arm no longer hurt but the under eye lid twitch is still occurring and at the same frequency.

Patient with longstanding bilateral non-pulsatile tinnitus without associated hearing loss. No dizziness, ear pain, hearing loss or vertigo.He received Moderna vaccine on 3/2 and his tinnitus got much louder. It improved but has not become louder again. He is currently treated with trial of niacin.

I had some type of allergic reaction in my eyes that caused them to burn, be sensitive to light, and felt grainy. This happened the days after getting both of my vaccines. The only thing that helped was resting my eyes but laying down caused them to burn more than compared to laying down. I could only keep my eyes open for a couple of seconds at a time but then they would start to burn. After those symptoms started to wear off, my eyes were sensitive to sunlight for more than a week.

Member presented with profuse sweating and said he did not feel well at all. Because I do not have aassigned to the clinic, I went to ask an to do a physical examination on the member. The went retrieved water for the member to help him cool down and another patient was in need of my assistance. As I returned I saw the conducting his physical examination of the patient where he started to examine the member where the member did not present with any hives on his body but his tongue was slightly larger than normal and he started to clinch his stomach and he could not physically drink the water and he then fell over gasping for air and eyes bulging(you could hear him

patient walked to the observation room independently and about 5 mins after arrival to the observation room, he become very pale, eyes rolled back, and appeared to have a seizure type of activity. Patient was transferred to a cart with assistance, was able to respond to commands, and was transported to the Emergency department.

Patient sat down to wait 15 minutes after the vaccination. other patients observed that he appeared to fall asleep then his eyes rolled back in his head and he went rigid and his body flexed straight out. another patient prevented him from falling out of the chair. i went out immediately to check on him and he came out of it. it lasted about 30-45 seconds. his color was not good so i instucted my technician to call 911. he was taken to the store directors office and the paramedics examined him and said everything appeared normal. he suffered no more reactions.

Tightness of right side of jaw. Difficulty chewing and pain when light pressure is applied. Pain extends along jaw to right ear

After about 10 minutes of sitting in observation, patient reported his tongue was tingling. Gave patient benadryl to treat symptoms. at about 4:51 he reported his face was tingling. Checked injection site and it looked normal, no redness. We continued to monitor patient until 5:30 than the Patient stated he was starting to feel better and had arranged a ride home and someone to stay the night with him and left the pharmacy.

Symptoms of mild COVID. Loss of taste, smell starting 4/10. Fatigue and muscle soreness starting 4/9, at approx 3 PM. Nasal congestion starting 4/9 early AM. Chills, fever 4/10.

PT started feeling dizzy , seeing a blinding light, turned pale and started sweating felt like passing out

Pain in the left ear (same side of shot). Pain in the ear is in the canal area (so below/behind the ear itself) and in the neck on the side of the shot near my lymph node. I received the shot in my left shoulder. This and soreness at the shot site lasted for 3 days. The ear pain has now come back after a week. I also experienced general fatigue for 3-4 days after the shot.

Patient complained of feeling anxious a few minutes after receiving the vaccine, while he was just about to walk up stairs to the observation area. . Stated, "I could not see, my eyes went black, and my fingers were tingling. I almost passed out". Clinician immediately sat patient on a chair and gave him water, which he drank. Medics were called to see patient. Vital signs were WNL. Once cleared by medics, patient was escorted to the ground level observation area. He soon reported that he was back to baseline. Medics cleared him as being medically stable. Patient reported that he no longer was light headed and had no visual disturbances. {Patient did say that he often gets anxious with needle injections.

Extreme headache and photosensitivity on 4/16/21. Spot in central vision, blurry vision in periphery, eye pressure and headache on 4/21/21. Following up vision issues with an opthalmologist.

Probable vasovagal syncope. I had no anxiety up to and during the vaccination. Immediately after being vaccinated, I grew dizzy and felt my vision fading. I asked for some water and waited. Three minutes after vaccination, I lost consciousness and forcefully fell on the floor, scraping my head and biting the front of my tongue. I was immediately wheelchaired over to a nearby bench and tended to by a nurse and two doctors. I was sweating profusely, had a low heart rate and chills, and felt tired. After forty-five minutes, a bottle of water and a can of orange juice, I had recovered and was dismissed. I already had a ride waiting to take me home.

A tick/twitch in the right eyelid that started shortly after shot. Is somewhat on and off and off but does consistently come back.

The patient developed high fever to over 103F on the day of vaccine. 2 days later, he developed a red rash on the right side of his forehead, and in the next 4 days, swelling of right side of his face, right cheek, right temple, mandible, right ear, with difficulty of chewing, and some decrease in hearing in the right ear. The patient never had dermal fillers. I saw the patient today and put the patient on Prednisone and Keflex empirically, and referred him to dermatologist and plastic surgeon for 2nd opinion.

At approximately 12:45pm on 4/27/2021, patient received his first dose of Covid19 (Moderna) vaccine, given by Pharmacist on duty. During the 15 minute observation period following the vaccine, the patient's father asked the pharmacist to check his blood pressure. While the pharmacist was checking his blood pressure (approximately 2-3 minutes following the vaccination), she notices the patient was starting to look pale and shut and squint his eyes as if he was having trouble focusing his vision. The pharmacist questioned the patient to make sure he felt ok, and the patient said his vision was going black and he was getting dizzy and lightheaded. The pharmacist moved nearby objects out of the way to prepare to lay the patient on the ground. When returning to the patient, he was staring blankly, hunched over, unresponsive, no pulse felt on a 5 second check, and had rheum dripping from nose and eyes. At 12:50pm the pharmacist instructed technician to call 911. Patient was lowered to the floor in the supine position where he remained unresponsive and no pulse could be felt on a 10 second check, but chest rise could not be evaluated as patient started to have convulsions. Convulsions ceased after approximately 15 seconds and patient started groaning and moving his head and said he could hear but he couldn't open his eyes yet. Patient remained supine and the Pharmacist continued to carry on conversation with patient until EMS arrived at 12:57pm to evaluate patient. EMS attributed the instance to syncope related to the vaccine. The patient denied ambulatory care to the hospital for any further evaluation. Patient denied history of fainting expect for ?one unprovoked episode at his grandmother's house when he thinks his blood sugar got too low.?

Anaphylaxis / anaphylactic shock, hot feeling in throat (a few minutes after receiving second dose), tightness in throat, difficulty breathing, hyperventilation, shallow breathing, wheezing

About 5-10 minutes after the shot was administered, patient complained of some dizziness, nausea, lightheadedness. Few minutes later. some tingling of the lips. Patient was given about 50mg of diphenhydramine about 10 minutes after the shot, some water to drink and his blood pressure was checked. BP was 114/78 and heart rate was 68. Patient was allowed to sit for about an hour and felt better over that time and was able to go back to work cautiously

Shortly after receiving the vaccination the patient began to feel light headed. He stood up and fell back to the floor, landing in a sitting position and not striking his head. He was helped into a chair by a bystander and I was alerted to the situation. He appeared pale and diaphoretic, then he became unresponsive with his eyes up and his arms tensing up. After about 10 seconds he became responsive again. EMS had been called earlier and arrived a few minutes after this and he was evaluated and left with EMS. It was unclear if he had simply passed out or if he had a seizure. He had no previous history of seizure, though the presentation seemed somewhat atypical for just passing out.

Patient felt like fainting about 10 mintues after dose. Sweating, ears ringing, . Patient fell as a result of dizziness and suffered a few lacerations to arm, bumped his head and complained of pain. 911 was called and patient was sent to Emergency room per ambulance for a checkup.

My tinnitus worsened significantly. Before, I could only hear it in a completely quiet area. Now, I can hear it over most noise except for very loud sounds.

client complained of dizziness and headache after receiving 1st dose Moderna. Client had already completed 30 min observation, had walked outside to vehicle, felt dizzy and returned back to EMT station. EMT took vitals at 1635: blood pressure 148/104, pulse 74, oxygen 100%. RN and PHN joined EMTs. Blood pressure rechecked to confirm first value: 148/104. Per client had just had coffee, lunch and no hydration(water) all day. Per client history of asthma 10years ago, no medications, no known allergies. Vitals 1645: blood pressure 141/91, pulse 66, oxygen 99%. Per client feeling nauseous and blurry vision. EMT gave emesis bag and water. Vitals 1650: blood pressure 138/87, pulse 62, oxygen 100%. Client stated nausea resolved. At 1653 client stated headache worsened. At 1700 client stated vision improving. Vitals: blood pressure 138/90, pulse 75, oxygen 100%. Per client all symptoms had resolved. Client left facility with steady gait at 1735 after completing 1hr observation. At 1746 client returned to EMT station, stated once he began to drive he felt nauseous again. Vitals at 1750: blood pressure 148/90, pulse 65, oxygen 98%. Vitals at 1756: blood pressure 129/92, pulse 70, oxygen 98%. Per client nausea about the same. Vitals 1800: blood pressure 133/95, pulse 64, oxygen 98%. Per client nausea slightly improved, light headed, and slight vision changes per client. RN Thong offered client paramedics for further assessment, client denied, would like to stay longer in observation. Vitals at 1809: blood pressure 140/92, oxygen 72, oxygen 99%. Per client feeling hungry, still nauseous. 1820 vitals: blood pressure 136/98, pulse 66, oxygen 96%. Nausea and vision improving. 1831 vitals: blood pressure 134/87, pulse 69, oxygen 98%. Per client vision same, nausea improving. Client had family member come and pick him up. RN advised client to follow up with provider regarding reaction. ER precautions also given. Client left facility with steady gait at 1840.

Vaccine administered 8:32am. At 8:36am VS BP 74/51, P 40, R18 T98. pt sitting on chair and fell on right lateral side. Pt noted with bump to right forehead. Pt A&O verbalizes feeling dizzy and nervousness. Pt diaphoretic with brief episode of seizure-like eye twitching. Pt denies any chest pain. 8:39am VS BP 90/40 P50, R18, T98 O2 Sat 98%. Lead nurse made aware. Md on call made aware. As per MD notify EMS. Orders carried out. Applied cold compress to forehead while pt sitting up. Wife present. VS 8:42am BP 128/76, P74, R18, T96.8, O2 98%. Pt under no distress. 8:52 EMS arrived to site. Pt evaluated by EMS and offered to be taken to hospital for further evaluation. Pt refused. Advised pt of consequences for leaving AMA. Pt verbalizes understands. 9:01am pt signed AMA. Instructed pt to seek further medical assistance if signs/symptoms persist at home. Assited pt to exit site on wheelchair accompanied by wife.

PT COULD NOT HEAR OR SEE AFTER VACCINE . HIS BLOOD PRESSURE WAS DROPPED . i WAS CHECKING HIS BLOOD PRESSURE EVERY 5 MINS. H E WAS REALLY HUNGRY SO GAVE HIM SOME CRAKERS AND WATER. CHECKED AGAIN HIS BLOOD PRESSURE AND AFTER THAT HE WAS FINE . WE HAD MONITOR HIM FOR 30 MINS .

Covid Vaccine was administered at 11:52 AM. At 11:57 AM patient began experiencing right arm numbness, loss of vision, experiencing hot flashes and shortness of breath. Vitals were taken : Temp:: 97.8 BP: 90/60 mmHg Pulse : 48 Spo2: 81 % Glucose: 89 Notified provider Fenny Lin, FNP Provided Nasal Ammonia inhalant and Oxygen was provided to patient at 2 Liter per minute for 20 Minutes. Stabilized patient Provided Gatorade for hydration Observed patient for an additional 30 Minutes per provider. After 30 Minutes of observation Vitals were taken at 12:37 PM Temp: 97.9 Spo2: 100% Pulse:51 to 57 BP: 92/62 mmHg. Symptoms resolved and patient was advised to follow up with his PCP for additional assistance. Also notified if symptoms returned must go to nearest emergency room for further evaluation. He may also contact our office for any further concerns. Per provider okay to release patient.

The patient had his 2nd Moderna vaccine on 4/21/21. He had headache and malaise for 2 days that resolved. On 4/24, he developed chest pain radiating to his jaw and left upper arm. He presented to the ED, he had an elevated troponin of 2.24 increased to 12.87. EKG showed ST elevation consistent with myocarditis. He was transferred to RRH and underwent cardiac cath on 4/26 which showed no CAD. He was not febrile. He was treated with metoprolol

9:25 am BP 119/80, p- 75, o2 98%, c/o dizziness and vision blurred. Mint given. Left hand also numb. 9:34- 119/72, p-63, 02 100%DIzziness better, vision better. 9:41 sitting bp 118/76, no complaints, feels good sitting. 945- standing , bp 115/71, p- 70, o2 99%, "doing good". 9;50 vision fully returned, denies dizziness, nausea or numbness

Lost vision, lost ability to move, collapsed on floor, one side of body regained movement, dizziness, chest pain

24 hours after the second Moderna dose, I began to feel very weak with a strong headache and a low grade fever. By Thursday (48 hours after vaccination) the headache was very strong and by that night it was debilitating. The fever had increased as well to between 101-102 degrees. Thursday night Friday I treated symptoms with alternating doses of Ibuprofen and Acetaminophen, which succeeded in reducing the fever but did not rid of the headache. Saturday the debilitating headache persisted with extreme light sensitivity and nausea. The pain became so great Saturday night that my wife took me to the Emergency Room. There they performed a brain scan with an MRI that came back normal and treated the pain with Morphine. I was sent home around 8am Sunday morning with a prescription for Toradol. The pain did not decrease with the Toradol and by 6pm Sunday I was vomiting blood and the pain was excruciating. At the advice of my primary care doctor, I went back to the Emergency Room. In the Emergency department they performed a Lumbar Puncture and collected spinal fluid for testing, as well as blood for lab work. The pain was managed with Fentanyl. Late that evening the Hospital admitted me. I was given two more MRI's of the brain - one with contrast. I also underwent an upper scope of my esophagus, stomach, and upper colon which found 3 small ulcers likely due to the high volume of NSAID pain relievers taken over the previous days. The lumbar puncture revealed that my white blood cell count was 51 and they officially diagnosed aseptic meningitis.

The occurrence of additional non-serious events is detailed below. No Medical History information was reported. On 12-Apr-2021, the patient received first dose of mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) 1 dosage form. On 12-Apr-2021, the patient experienced MOTION SICKNESS (motion sickness in car) (seriousness criterion hospitalization), EYE MOVEMENT DISORDER (micro movement of the eye) (seriousness criterion hospitalization), OCULAR DISCOMFORT (lack of focus of the eye) (seriousness criterion hospitalization), DIZZINESS (prolonged dizziness) (seriousness criterion hospitalization), HEADACHE (intense headache) (seriousness criterion hospitalization) and HYPOTENSION (Dropping blood pressure) (seriousness criterion hospitalization). On an unknown date, the patient experienced MYALGIA (Myalgia). At the time of the report, MOTION SICKNESS (motion sickness in car), EYE MOVEMENT DISORDER (micro movement of the eye), OCULAR DISCOMFORT (lack of focus of the eye), DIZZINESS (prolonged dizziness), HEADACHE (intense headache), HYPOTENSION (Dropping blood pressure) and MYALGIA (Myalgia) outcome was unknown. DIAGNOSTIC RESULTS (normal ranges are provided in parenthesis if available): On an unknown date, Blood pressure measurement: mmHg (Low) Dropping blood pressure. On an unknown date, Cardiac stress test: (Inconclusive) Inconclusive. On an unknown date, Magnetic resonance imaging: (normal) had a cyst but nothing that would cause symptoms. The action taken with mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) was unknown. Treatment medication includes antibiotic for cyst, 25mg 3xday meclizine for inner ear, and Midodrine HCL 10mg for dropping blood pressure and 10mg Pregnisone for inner ear inflammation. Patient underwent VNG test had no inner ear inflammation and noticed micro movements of the eye No concomitant medication information were reported Caller wanted to report adverse event. Callers first dose was on 12apr21, LOT#040B21A, Expiration date 10/2/21. Caller said his life has been upside down since he got the shot. First week had intense headache and dizziness. Second week he had muscle aches and dizziness with limited headaches. Third week he is having a focusing and dizziness issue. Caller said he seen doctor and specialist. Caller had MRI and Stress test. Caller said MRI came back clean. Caller said they had a cyst but nothing that would cause symptoms. Caller said Brain scan came back negative. Caller said he was completely heathy. Caller said he also gets motion sickness now since he had the vaccine. Primary care doctor gave antibiotic for cyst. PCP thought had something to do with inner ear, 25mg 3xday meclazine, then the gave Midodrine HCL 10mg for dropping blood pressure. Caller said he got relief for 3 days then he started getting sick. Caller went to ER and they took him off the medicine. Caller when to a ENT doctor. Caller said took VNG test. He was prescribed 10mg Pregnisone for inner ear inflammation. The VNG test proved he didnt have inner ear inflammation. Caller said his symptoms have been going on for 32 days as of yet. CAller said he cant go to restaurants because of dizziness and sickness. Caller said during his VNG test they noticed micro movements of the eye. His ENT thinks this may be causing the dizziness and lack of focus. Caller said he will travel to anywhere to get help.; Sender's Comments: Based on the current available information and temporal association between the use of the product and the start date of the events, a causal relationship cannot be excluded. Further information regarding detailed medical history and diagnostics will help in assessment.

Pt said he was disoriented , lightheaded and dizzy. We had him sit to observe him. He was flushed all over body and cold to touch. He stated he was feeling nauseous. I led him to a private space and had him lie down. He stated that his lips were tingling. I gave him 25mg liquid diphenhydramine and continued to observe. He said throat was very dry and feeling worse. I contacted 911 and they came to the clinic to evaluate the patient. They checked blood pressure, blood sugar, heart rate and rhythm. At 5:25 he decided he was feeling a little better and he refused transport by the ambulance. We arranged for an emergency contact to come pick him up.

Pain at my jaw joint approximately one week after the second injection. I could not open my mouth without pain. Once I could open my mouth, I could not close it without severe pain. This continues to today but has become less severe of an issue.

Patient stated he could (taste) vaccine. Had patient wait awhile, said he was ok and was leaving. Counseled patient t go to emergency room if uncomfortable and had any other side effects.

about a month later i got a weird ringing in my ear I went to the Ent and doctors and they couldn't find any infection

Chills, fever, hive like rash all over body a week after, and smells and tastes are no longer the same. There is a certain taste and smell (which are the same) that quite a few foods and other things taste and smell like.
