7/7/21 @ 9 AM: ONSET OF HEADACHE FOLLOWED BY VOMITING. CHILDRENS TYLENOL GIVEN. 7/7/2021 @ 8PM: ONSET OF LOWER CHEST/ UPPER ABDOMINAL PAIN AND VOMITED 3 TIMES. WENT TO ER @ HOSPITAL @ 9 PM-11 PM HAD EKG & ZOFRAN GIVEN

Moderna vaccine given outside of recommended age range. Child felt feverish the next day only per mom.

Vaccine recipient had received a dose of Pfizer vaccine about 3 weeks ago at a clinic. Recipient present to this clinic with his mother on 6/25/21 for his second dose of Pfizer vaccine. The nurse discussed the Pfizer vaccine and accidentally grabbed a Moderna. She vaccinated the child with 0.5mL of Moderna vaccine IM. Immediately after the recipient was vaccinated, the nurse realized she had given the Moderna instead of the Pfizer. The mother was notified immediately, vitals were taken and the child observed for 30 minutes post vaccination. Child had no ill effects and was sent home with clinic phone number in case of any issues. A nurse from this facility contacted the parent on 6/26/21 to check on the child, mother reported that child had "a slight headache and his arm is a little sore". A nurse from this facility contacted the parent on 6/28/21 again to check on the child, mother reported no complaints of headache arm soreness, or any other complaints.

pt received 1st vax which was moderna and was supposed to be pfizer. on July 2, he recieved another shot of moderna instead of pfizer. he has severe headaches, nose bleeds, and weakness.

The patient received the vaccine and did not have a reaction until Tuesdays the 19th. He had fever and chills. His grandfather signed the consent and told the nurse that he need to have the vaccine because he has asthma. The pediatrician increased his breathing treatment and prescribed antibiotics due to an upper respiratory infect that he had previously. He is now stable and remaining in the home.

Recevied vaccine on 5/14 around 6 pm. Started noticing chest pain, chills and fatigue on 5/15 around 6 pm. Evaluated by ED on 5/17 subsuquently admitted to PICU with intermittent chest pain and elevated troponin in the setting of recent Covid vaccination as well as a history of WPW status post ablation with recent onset of intermittent tachycardia. EKG demonstrates nonspecific ST segment changes and has elevated troponin which likey points to myocarditis as a diagnosis. Continues with elevated troponin level, no medication intervention at this time, no longer having chest pain

Received second dose of Moderna COVID vaccine on 7/2/21. 7/3 developed fatigue, dizziness, and decreased hearing. Symptoms progressed to difficulty breathing and chest pain. 7/4 chest pain localized to left side of chest. He was seen in local ED with elevated troponin of 8154, EKG with diffuse ST segment elevation. Transferred to another ED for further evaluation. He received IVIG on 7/5 and did develop V-tach lasting about 7 beats around 0500 on 7/5 but otherwise hemodynamically stable.
