Left deltoid pain, swelling, redness, warmth; Swollen, painful, left axillary lymph nodes ?diagnosed with cellulitis 1/18/21 and given keflex every 8 hours x7 days.

Approximately 6 hours after the injection at 6 pm I began to experience right sided ankle pain and swelling and mild chills. The pain peaked about 2 hours later about a 5/10 and was constant. I took 200 mg of Ibuprofen and elevated my foot. The right ankle was visibly swollen and red compared to the left. Prior to going to bed the pain had mostly resolved. Upon waking up in the morning the pain had entirely resolved. There was some residual stiffness that resolved throughout the course of the day.

Moderate to severe axillary lymphadenopathy requiring missing work and inability to perform his functions as a first responder/wear protective gear

Rock solid lump over my collar bone possibly lymph nodes, it started 7-8 days after the shot and I still have it same side where I got the shot, 3 days after the shot I also got pain on my armpit . I?m worried about lump over my collarbone

Pt recived his second Moderna vaccine 2/15/21, and on 2/16, staff at pt's group home noted pt was unable to walk although pt previously ambulatory (could go on long hikes prior to acute illness). Pt was grabbing his legs and rubbing them, and initially staff thought it was pain that would resolve. However, on 2/22, lower extremity edema was noted, and pt without improving symptoms, so he was brought to the hospital for evaluation and treatment. GBS suspected, IVIG initiated.

?COVID arm? a 6 inch warm to the touch rash that is swollen and itchy/painful that began developing 2 days after receiving the vaccine

2 weeks prior to vaccination (12 Jan) patient was seen for 4yr L breast mass. Referred for breast ultrasound. Patient underwent COVID vaccination 04Feb. Noted approx 16Feb to have increasing swelling in axillary region. Pt underwent breast ultrasound on 23Feb found to have breast mass and atypical appearing axillary lymph nodes, recommended for biopsy. Followed up with provider 25Feb and was referred to General Surgery. 08Mar had initial surgery consult, symptoms had modest improvement, recommended by surgery to monitor symptoms and defer 2nd COVID vaccination. 13MAR - axillary lymph node swelling returned 15Mar - patient was recommended by immunizations provider and nurse to defer 2nd vaccination and return to surgery for likely excisional biopsy of axillary lymph node and breast mass. Awaiting final surgery input on treatment plan.

Rash on palm of hands 7 days after vaccination. Red spots only on palms with slight itch and sting Rash developed on soles of feet at the end of day 7. Same style of rash. Day 8 resulted in uncomfortable swelling and flush skin around rash. Rash is warm to touch. Rash is slightly itchy and moderate stinging when trying to close fist or hold items or walking on feet. Ibuprofen and Benadryl manage symptoms to the point they are tolerable but annoying.

I received the vaccine on 3/25/2021 at 5:30PM and had general soreness in the arm the injection took place. Soreness was remarkable until four days after the injection. Night of 3/30/2021 at approximately around 9:30 pm I felt soreness in the left axillary region (in my left armpit). This at first was thought to be unremarkable as I was wearing a tight shirt at the time. However, after switching to looser clothing the soreness and pain continued and now considered remarkable. The day after (3/31/21 Today of this report), I still have pain and what feels to be a swollen lymph node in the left axillary region (in my left armpit). This is sore and painful and feels palpable.

Itching and redness to L arm 7 days after vaccine, localized lymphadenopathy. No difficulty breathing or swallowing

I received my first Moderna shot on March 24th. Just had a sore arm the next day. April 2nd: Around 1pm at work I was starting to feel a headache coming on. By about 2-3pm it was worse and felt tired/drowsy. I ate supper then went to take a nap at 7:30pm. I woke up at about 10:30pm, checked the time and just felt out of it so I went right back to sleep. April 3rd: I slept until my dog woke me at about 5:50am and felt pretty good, still minor headache and some chills. During the night I did feel some soreness in my arm that got the covid shot. After I showered I checked my arm and it was swelled up with a rash like redness maybe an inch in diameter. I was fine for most of the day enjoying the nice weather outside. Then about 6pm I started to feel the headache again and some chills. Went to bed at about 10:30pm, headache was bad, had the chills, started coughing some, and just didn?t feel normal, took some Tylenol to help through the night. April 4th: Slept through the night fine, woke up with a mild headache and a sore throat. Had some dry coughs occasionally throughout the day, but was fine most of the day. I did check my temperature throughout these past few days and all check were normal. April 5th: Almost back to normal, still have a sore throat and cough. My arm rash near the shot area has gone away, just a little swollen. April 6th: Back to normal health.

Five minutes after receiving dose two of current vaccine series, this patient developed hives near bilateral wrists and a growing erythematous area around vaccination site. Site is warm to the tough and slightly swollen, reaching to lymph nodes near left armpit.

received shot at noon thursday. 8 hours after shot I started feeling my jaw get very sore and tight. went to sleep, woke up constantly during night fluctuating between being hot and being cold. woke up and had body aches and left arm was very stiff and sore. spine pain and headaches for most of the following day, which significantly subsided when I took Ibuprofen at ~5pm. went to sleep, woke up saturday feeling almost perfectly fine with very slight headache. noticed that i had a swollen bump on the direct left side of my neck that was tender to touch and felt muscle related. still swollen at 8pm saturday.

3/31/21 26YM admit with Acute issue: myocarditis, chest pain, extreme fatigue, body aches, SOB; Moderna (first dose on 3/28). No known prior COVID. Patient admits that approximately a month and a half ago, he was drinking a significant amount of alcohol with a 24-pack of beer daily and 5 drinks at night. He quit this approximately 2 weeks ago when he got back together with his girlfriend. He has had stress in his life with the death of a cousin approximately 2 weeks ago. The patient had been feeling well over the last week or so and received the Moderna COVID-19 vaccine on 3/28 Sunday. This was his first dose. Approximately an hour and a half after receiving the vaccine, he had symptoms of feeling dizzy with position change. He went home from work. He developed fevers, chills, rigors, diaphoresis, and headaches. He slept a lot due to these symptoms. On 3/30 Tuesday, he woke with mild chest discomfort that he described as a pressure sensation in his anterior chest. He states that it was bothersome, but not too significant. He woke up 3/31 at approximately 0200 hours with a stabbing left anterior chest discomfort. He notes that it felt like a hot knife stabbing him. He admits that over the last couple of days, he had a cough when he would lay back in bed. He admits to feeling more short of breath when flat and feeling like he had to put pillows behind his head. He came to the emergency department due to these symptoms. He was given a dose of acetaminophen and promptly had nausea and vomiting. He received a dose of aspirin 325 mg and states that his symptoms resolved shortly after that. 3/31 He now feels well and denies any further chest discomfort. He denies any dyspnea.

4/26/21 Shortly after the shot, I felt funny the whole day. 4/27/21 Sore left arm. 02/02/21 Left arm pit's lymph node is swollen, left arm/hand loss of feeling. Feels tingly. Permanent damage? Maybe, hopefully not. I'm checking the box anyway.

PT CALLED TODAY (5/9/21) TO ASK IF HE WAS HAVING A NORMAL REACTION. HE THEN STATED HE HAD A RASH ALL OVER INCLUDING HIS FACE AND INSIDE HIS MOUTH, EVERYTHING HURTS, AND HIS FEET WERE SWELLING. I INFORMED HIM TO GO TO THE HOSPITAL IMMEDIATELY.

Systemic: Dizziness / Lightheadness-Mild, Systemic: Flushed / Sweating-Mild, Additional Details: Patient received COVID vaccine at 10:00am. Patient presented with symptoms of lightheadness and sweating within the 15 minutes of monitoring. Patient given water and stayed about 15 minutes more (30minutes total). Pharmacist on duty offered to call 911 but patient said he was feeling better. At 6:00pm mother called pharmacy and said son went to ER due to difficulty swelling.

Unilateral axillary lymphadenopathy on the left side of the body, inflammation at injection site, muscle aches, fatigue. Moving the left arm causes moderate discomfort due to tenderness of lymph node. Symptoms were most severe for the first 4 days after onset and seem to be slowly receding after that point.

1 day after vaccination developed new HA and nausea 2 days after vaccination developed new SOB, pleuritic chest pain

Concomitant products included VITAMINS NOS for an unknown indication. On 06-Apr-2021, the patient received first dose of mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) 1 dosage form. On 13-May-2021, received second dose of mRNA-1273 (Moderna COVID-19 Vaccine) (unknown route) dosage was changed to 1 dosage form. On 13-May-2021, the patient experienced SWELLING FACE (face puffy), RASH (Rash on his throat) and MYALGIA (body aches). On 14-May-2021, the patient experienced PYREXIA (fever of 101.3). On 14-May-2021, RASH (Rash on his throat) had resolved. At the time of the report, SWELLING FACE (face puffy) and MYALGIA (body aches) had not resolved and PYREXIA (fever of 101.3) outcome was unknown. DIAGNOSTIC RESULTS (normal ranges are provided in parenthesis if available): On 14-May-2021, Body temperature: 100.3 degree fahrenheit High. Treatment information was not provided. Appendicitis - Started not feeling good a little before dinner time. Stomach pains I thought were IBS related. I fell asleep and kept waking up with stomach pains worsening. Around 12:45 AM on 04/29/2021 I drove myself to the Hospital and by around 3:30 AM I was told that I needed to have my appendix out. I did not have any symptoms prior to having the vaccine. I was transferred to another Hospital to have surgery around 10 AM on 04/29/2021 to remove the appendix. The surgery was successful. I was sent home the morning of 04/30/2021.

Sore throat and swelling, (gave penicillin) Found later tonsils were swollen and was given Dextromethadone for it. The swelling went down immediately but came back after two days and then went down again over the course of a week.

Experienced fever, body aches and chills 3 days after receiving the 2nd dose. Around 9 o'clock in the evening I started having palpitations. Heart rate reached 170 bpm, tracked by my apple watch. Went to the ER. I was diagnosed with sepsis. Fever was 102.7, HR wouldn't drop below 105bpm. Procalcitonin and lactic acid were all elevated. I was given 5 liters of fluids that night. Was then started on Vancomycin and cefepime the following morning. I was in the hospital for 2 nights.

Following administration of the vaccine, the patient and his wife were seated and I requested they remain seated for 15 min after because I had recently had someone with a fear of needles collapse after their shot, at which point I had already counseled on the things to expect afterward but the patient stopped responding and slumped over. I was unsure if he was breathing as his wife was cradling his head, so I shouted for a technician to call 911. I heard a sound that I perceived to be wheezing and administered an epipen at which point the patients limbs began to contract so I then suspected it was a seizure. We lowered the patient to the ground and shortly after he became responsive. He was able to state his name and date of birth and breathing was unlabored. Pulse ox was 98 and HR was 88 BPM before EMS arrived. They examined the patient and determined it was "anxiety induced response" but also noted that the bloodpressure was "very low" which is consistent with anaphylaxis, but I did not examine the patient at that time. He was sent home afterward.

Pt is a 26 yo M w/ a PMH of COVID 19 infection (resolved), seasonal allergies, and head trauma (2019). He arrived at Facility on 6/15/21. On 7/8/21, pt had his tooth extracted (#30) at 0930 without complication. At 1200 same day, he received the first dose of the COVID-19 Moderna vaccine. At 1323, the pt was found lying on couch in dorm c/o pain in dental area, dizziness, and lightheadedness (BP 150/77, HR 91, RR 18, O2sat 99%). Heart sounds normal, breath sounds absent on auscultation. Pt was confused and in distress. (unable to respond verbally). His eyes were red and tearing. Tongue and lips appeared red and were mildly swollen. RN was unable to visualize throat in dorm. Epipen was administered at 1340 which resulted in an immediate decrease in symptoms. Pt was calmer and in less distress. Normal uvula with some narrowing of airway noted. He was then taken to clinic and was calm but shaking with an open airway and mild swelling noted; VS wnl. He left via EMS in stable condition to ED at 1400 for evaluation. Pt returned from ED same day in no acute distress but still c/o dental pain (5/10). He was last evaluated by provider on 7/9/21 and is asymptomatic. Denies swelling, SOB, or fatigue. No bleeding or swelling at dental extraction site noted. Pt evaluated by dental on 7/12/21. No swelling, erythema, exposed bone, or signs of infection noted. Area appears to be following normal post-operative course. Pt remains asymptomatic. ED summary- Medical Center (7/8/21): Pt sent to ED for allergic reaction/near-fainting (vagal reaction); Assessment: ?Most likely the syncopal episode was due to vasovagal syncope. Cannot rule out allergic reaction to the COVID 19 vaccination?. Drug screen negative. Discharged on 7/8/21.

Prolonged swelling with erythema lasting longer than a week and headaches that turn into migraines daily. Icing and or heating pad to aid in pain and swelling at injection site. Tylenol and Ibuprofen to treat the head aches that don't turn into migranes and migraine medicine (OTC- acetaminophen/caffeine tablets) and sleep to treat migraines.

On day 9 post vaccine, he developed a warm, red rash on his L deltoid. It was swollen compared to the other deltoid. It was redness 2x2 inches on L deltoid and itching on L upper extremity with one localized bump on L forearm. On 1/10 at 1 AM, he took Benadryl and applied hydrocortisone to the area. Upon waking up on 1/10 at 1 pm, his itching and rash had spread to both arms, back of neck, ears, and various spots along his forearm. He is having some irregular breathing, but could be due to nerves.

fever of 103.5 for 3 night, fatigue, swollen lymph nodes in left axilla. then HA, body aches and diarrhea. Seen at Urgent Care prescribed Keflex

Arm swelling and soreness at vaccination site onset 1-2 hours after administration. Chills and body aches with onset about 12 hours after vaccine that continued until following morning. Headache off and on with chills that persisted up to 24 hours after injection.

Hard, swollen, tender lymph node below left collar bone. First noticed 1/28/2021 but also had minor full body achiness for 2 days prior.

Had significant swelling and pain starting 4 hours post vaccination. Swelling subsided about 72 hours later. Now 8 days later arm is significantly swollen and itchy.

within one week of receiving the first dose of Moderna on 12/23/2020, both armpits became swollen. The swelling in the right armpit appears to have subsided. the left armpit still remains swollen 41 days later. i received the second shot of Moderna on 1/20/2021.

6 days after the first dose i've noticed swelling of lymph nodes. Supraclavicular lymph nodes are swollen and sore to touch as well as swollen lymph nodes under my left arm pit. The Calvicular lymph nodes is also located on the left side. The same as the injection point.

AFTERNOON POST VACCINATION PT EXPERIENCED LIGHTHEADEDNESS AND FATIGUE AND DIARRHEA, SYMPTOMS PROGRESSED AT NIGHT TO COLD SWEATS, FEVER. NEXT DAY PT FELT MORE DIZZY AND LIGHTHEADED, SEVERELY FATIGUED, DIFFICULTY BREATHING, SWOLLEN LYMPH NODES, BODY ACHES, PAIN AT SITE OF INJECTION. SWOLLEN TONSILS AND DIARRHEA. SYMPTOMS WORSENED THROUGHOUT THE DAY UNTIL PRESENTING TO THE ER, VITALS WERE ABNORMAL INCLUDING FEVER OF 101.8, TACHYCARDIA, RESPIRATION RATE OF 22 PER MINUTED, BP ELEVATED. PT WAS TREATED WITH MOTRIN/TYLENOL, IV FLUIDS WITH NO IMPROVEMENT OF SYMPTOMS POST DISCHARGE FROM THE ER. DAY 2 POST VAX STILL EXPERIENCING THE SAME SYMPTOMS AND PT STARTED DEVELOPING ULCERS IN THE MOUTH. PT SEEN IN ER SECOND TIME FOR SAME SMYPTOMS NOT IMPROVING, WAS GIVEN IV FLUIDS, TYLENOL, MOTRIN, THROAT LOZENGES AND NUMBING MOUTH WASH. ULCERS HAD TO BE CAUTERIZED THE WEEK FOLLOWING THE INJECTION. OVERALL SYMPTOMS LASTED APPROX. 2 WEEKS

On 2/21/2021 approx. 5PM patient noticed some pain and swelling of right axilla. Described as if "someone punched me in the armpit." Symptoms worsened by the next morning, so on 2/22/2021 patient sought care at a local Urgent Care facility. Treating provider felt symptoms were most consistent with cellulitis of the right axilla. One week course of antibiotics (Bactrim 1 tab twice daily) was prescribed. Follow up with PCP was recommended for 2-3 days or sooner if symptoms worsened or additional symptoms presented. No laboratory testing or radiology procedures were performed. Patient filled prescription for antibiotics and started taking same day. On 2/25/2021 patient reported that symptoms were improving but not completely resolved.

Adverse events were more applicable in the second dose of this vaccination. At 22:00 a low-grade fever began. At 03:30 on 3/3 a fever of 102 degrees had been achieved. Left axillary lymph node swelled. Generalized body aches mostly in wrists and ankles occurred. For the duration of 3/3 102 fevers persisted while taking Tylenol and slept for duration of day. On 3/4 low grade fevers of 100 degrees were maintained. Nausea and diarrhea occurred, aches continued throughout body particularly in shoulders. Lethargy so severe couldn't stay awake for more than 20 minutes. On 3/5 fevers reduced down to 99 degrees. Vertigo occurred with dizzy spells and ataxia. Heart rate periodically elevating to 120-140s for hour long durations. Only able to sleep for 2 hours at at time because of waking up from tachycardic episodes. Panic attacks ensued due to tachycardic episodes. On 3/6 psychotic thoughts and hallucinations began. First, racing thoughts about irrational an incontrollable events began. "Will I be able to father children?; I will sell my car tomorrow to sustain myself.; etc." Visions of insects crawling down a mountain side, flowers with eyeballs, etc. Panic so severe couldn't close eyes and mind always racing.

Symptoms of a fever (chills, sensitive skin, headache, dizziness) beginning ~12hrs after the injection and subsiding ~ 24hrs after the injection. 48hrs after the injection began to experience ?Covid arm?. Red swelling that is warm to the touch and slight itchiness. Extends around from the tricep to bicep and mostly below the injection site, down to near the elbow. No pain besides that of a normal IM vaccination. Currently still experiencing the symptoms, but not taking any treatment as none are necessary.

An hour after the shot he noted neck pain which was intermittent, the next morning he noted his throat feeling like it was closing up. Has had trouble swallowing since then now for 6 days with feeling of swelling under his tongue, no SHOB, but he feels weak. Diagnosis is mild angioedema treated with loratadine and prednisone taper begun the date of this report.

Fever-like symptoms over night the same day as vaccination including hot flashes and cold sweats. Pain and swelling in the receiving arm as well as minor swelling of the lymph node on the same side.

During my first vaccine I experienced a bruise on my arm that lasted ~3 weeks. My arm swelled (including my fingers which looked like sausages) for a few hours that night. I also felt like I experienced slight chest congestion for a week or two after. The second vaccine (lot number listed on this form) I have a bruise that in my opinion is worse than the one from the first shot. The bruise is still present after 1 week. I also feel slight tenderness around the joint area. I have not heard of these symptoms being advertise Will this cause joint damage? Additionally form both vaccines I experienced fever, body aches and obviously a sore arm.

I have developed a very pungent body odor, primarily in my armpits since receiving the first dose of the vaccine. The date I entered for symptom onset is an estimation. I do not recall when it began, but it was about 2 weeks after the first shot. I have been dealing with body odor and my armpits still feel swollen compared to before the vaccine.

Myocarditis. Pt received 2nd Dose Moderna on 4/21/21 morning. He reported developing fever of 102 F on 4/22/21. He reported developing chest pain early morning on 4/23/21. Denied other symptoms. Pt went to urgent care due to chest pain and SOB. Pt sent to ED from urgent care due to abnormal EKG. EKG showed ST elevation. Troponin I elevated 13.0 . C-reactive protein elevated 8.22. Left heart cath done, normal. CT PE done to rule out PE. Pt diagnosed with myocarditis and severe inflammation. "Elevated Inflammatory markers possibly related to COVID vaccine."

Pt experienced malaise, sibjective fever, chills, headaches, and myalgias following vaccination then on 4/11 (3 days post vaccine) experienced sharp chest pains radiating up left side of neck and down bilateral arms. Pt presented to urgent care clinic where EMS was activated for transport to the Emergency Department. Pt was evaluated for STEMI by cardiologist in the ED, cath lab was not recommended and pt was admitted for ealuation of possible myocarditis.

Patient received second Moderna on 4/27/21. Next day had chills, body aches, felt hot, but did not take his temperature. Those symptoms are gone nowm but woke up today with swelling on both sides of his nose, on his face. No congestion or breathing problems per patient. He has appointment with his provider this afternoon.

arthritic pain in fingers, toes, hip, knees. Painful and swollen, unable to close hands or feet. Pain and swelling subsided over 4 days. Some bruising on toes under the nails after swelling and pain subsided. Trouble sleeping

A rash appeared on my upper right hip that itched quite severely at first, but no longer does. Still, the red dots remain.

I first noticed I was itchy on the evening of 5/11 and noticed hives on my body the next morning. They continued to get worse for several days and appeared on all parts of my body from head to toe. I noticed my feet hurt and that I was showing signs of angioedema beginning on 5/14 and peaking the next day. I also experienced joint/tendon pain in the evening of 5/16 between my legs and feet (upper ankle). I still have hives today. The hives were particularly bad in high-touch areas, near the armpits, lower arms, pelvis areas. I took Benadryl occasionally which helped but at no point did the hives go away.

Swollen lymph nodes appeared near collar bone on side of injection site within 72 hours of receiving first Moderna vaccine injection. Lymph node swelling subsided within 10 days of injection.

The patient received the vaccine on 06/1/21 and claimed that a bump formed later which remained for 4 days at which point he claims that it had moved from his injection site to the left side of his neck under the surface of the skin where it still remains. He said that it wasn't painful at all, there was no visible redness or swelling upon visual inspection but he claimed that he could feel it. He was instructed to go get the lump checked at the local hospital on 6/20/21, the day the event was reported to the pharmacy. The patient has no primary provider that he sees.

Patient is a 27 year old male with history of allergic rhinitis. He moved to the area in April 2021 and had some initial problems with pollen allergies, but had improved and doing well with Singulair. He had COVID19 in November 2020, recovered fairly quickly and did not have much in the way of breathing difficulties. Last month he received both doses of Moderna COVID19 vaccination, second dose administered on 5/25/2021. After his second dose, he has become progressively more ill. He did have a fever up to 105F for 24 hours afterwards, but this resolved. He has gone to several urgent cares due to progressively worsening SOB, cough, and malaise. Coworkers have commented to him that he is just not himself and is slow to get around. He has gotten to the point he cannot take the trash out without having to stop to catch his breath. He has been treated with a variety of regimens, including amoxicillin, doxycycline, inhalers, prednisone, and IM steroid shots. He does get brief mild relief with albuterol inhaler.

Patient reported lump near collarbone about "3/4 size of a golfball" that only hurts when pushed. Developed on 7/2/21 in the evening

Had numbing pain on right side thumb finger. It got worse and got swollen after some physical activities after 5 days from day of vaccination

Left arm and hand with tingling sensation which spread to right arm and legs the following day. no overt rash. Intermittent bilateral ear pain. started 3 days later lasting 20 minutes to an hour then resolves then recurs. seen in ER 7/18/2021 for this, benign work-up.

Supraclavicular lymph node (mobile, initially mildly tender, 1-2cm, soft) occurring on left side (same side as the vaccine). This was first noted 2 days after vaccination. Associated symptoms including tenderness at the left deltoid (injection site). No fevers, fatigue, night sweats noted.

Injection site is sore, raised, swollen/firm and warm to the touch. He also reports a swollen lymph node in his armpit. He feels the soreness ebbs and flows.

Fevers, Chills, and facial swelling with previous facial fillers. Treatment: tylenol, benadryl & steroids.

1/26 Headache; 1/27 headache, body ache and fatigue; 1/28 fever blister on lip, eye and eyebrow; 2/1 chills, body ache, elevated temperature 102.7 went to the ED

Pain in arm within an hour, swelling in arm within 3-6 hours; pain and swelling started to subside within 36 hours; rash at injection site developed between 36 and 48 hours of injection. Sudden onset of intense chills/shivering and loss of appetite 12 hours after injection; fever symptoms started to subside after 12 hours of onset.

Received Moderna first dose on 3/11/21. Experienced sore arm and drowsiness same day. One week later (Thursday, 3/18/21), sore arm and slight muscle/joint pain came back and has subsided. Friday evening I experienced left jaw and ear pain and has subsided. Saturday morning, small bumps/rash appeared on left forearm (vaccine in left shoulder). Saturday evening, same thing on right forearm. Bumps/rash are similar to blisters and some have pustules. Itchy and painful. Took Benadryl. Sunday morning, more on hands, feet, and ankles. Continued Benadryl and monitoring. Sunday evening, increased density of sites on hands, feet, and ankles. Some showing on lower legs and knees. Monday morning (now), itchiness and pain increased. Increased Benadryl intake. This has helped reduce itchiness slightly. Sites are still painful. Hands feel slightly swollen/tight where sites are.

Pain and swelling in left armpit, followed by chills and trouble sleeping on the first day. Fever of 101.5, extreme fatigue, chills, headache, multiple swollen lymph nodes, aches all over body, for the following 4 days and continuing presently.

Mild fever first day. Second Day I had two inflamed lymphnodes under my armpit and near my neck with medium to decent pain.

I?ve been experiencing inflammation at areas of direct contact. It started about 20 days after the second dose of moderna vaccine. Symptoms started in my feet. About one week later it transitioned to my arm. Then to my face. Then to my buttocks. Then to my testicles. These symptoms happen after direct pressure is applied to an area for an extended period of time and last about 1-3 days. I?m am a resident doctor and needed to go to the ER when my face had angioedema. I?ve gone to my primary care physician as well, who agrees that these symptoms could be vaccine related. I have photos and time stamps of all the above mentioned symptoms and episodes

Had headache a few hours after injection, injection site hardened and had a rash, a day later I developed a high fever. My face swelled up and it was hard to breathe for a few hours. On the third day the swelling stopped but the fever took a few days to go away. It's been 2 weeks and a painless rash is still at the injection site.

PATIENT STATES ON 4/27/21 (THE DAY AFTER) THAT HE'S FEELING FINE TODAY WITH NO MORE HIVES. STATES CONCERN REGARDING HIS SECOND DOSE AND THINKS HE MIGHT NEED TO GET SECOND DOSE AT MEDICAL FACILITY IN CASE SOMETHING MORE SEVERE OCCURS. PATIENT STATES HE JUST WANTED TO REPORT AND LET PHARMACY KNOW.

Patient states after receiving the 1st dose a few minutes after he felt like his lymph nodes in his throat were swelling and he had some mild difficulty breathing and swallowing throughout the day, but didn?t get worse and resolved the next day. Denies any wheezing, SOB or rashes. Patient did not inform provider of symptoms because he felt they were not getting worse at the time and was not in any distress. Patient was briefed of severe reactions to notify provider of immediately and did not follow instruction.

He reports that he had his second Moderna COVID vaccine was 4/20 Tuesday. The next day, on Wednesday he began having headache, body aches, chills, fevers and mild shortness of breath. Patient reports that the body aches somewhat resolved on Thursday but he continued to have some intermittent palpitations. He woke up Friday with self described chest discomfort with persistent palpitations.

The occurrence of additional non-serious events is detailed below. No Medical History information was reported. In April 2021, the patient received first dose of mRNA-1273 (Moderna COVID-19 Vaccine) (unknown route) 1 dosage form. On 17-May-2021, the patient experienced VOMITING (vomiting). On an unknown date, the patient experienced BELL'S PALSY (might have what looked like bells palsy on one side of his face) (seriousness criterion medically significant), PRODUCT DOSE OMISSION ISSUE (second dose appointment was scheduled for 15May2021 which he missed), NERVOUS SYSTEM DISORDER (neurologic spasm up and down his spine), MUSCLE RIGIDITY (serious rigid back in the middle of his back) and LYMPHADENOPATHY (swollen lymph nodes along his neck/ has swollen lymph nodes). The patient was treated with CANNABIS at an unspecified dose and frequency. At the time of the report, BELL'S PALSY (might have what looked like bells palsy on one side of his face), PRODUCT DOSE OMISSION ISSUE (second dose appointment was scheduled for 15May2021 which he missed), NERVOUS SYSTEM DISORDER (neurologic spasm up and down his spine), MUSCLE RIGIDITY (serious rigid back in the middle of his back) and LYMPHADENOPATHY (swollen lymph nodes along his neck/ has swollen lymph nodes) outcome was unknown and VOMITING (vomiting) had not resolved. Approximately 7 days after vaccination, patient experienced a rash and swelling on right leg. He was seen by his physician, and MD recommended Benadryl to help relieve symptoms which resolved after an additional 7 days.

Patient reports bilateral leg swelling, temp 102, 1-2 days after COVID vax. He looked up DVTs, found that they are usually unilateral. No medical care sought. Swelling now resolved. Fever resolved.

I went to the doctor for Swollen Lymph nodes under the left armpit and lower abdomen pain. They scheduled an ultrasound of my left breast and a CT of my lower abdomen and pelvis. I have a 3-month follow-up for the lymph nodes and they found minor prostatitis in my abdomen. I was proscribed CIPRO for prostatitis. I then had a rare reaction to the CIPRO which caused my Achilles and patellar tendons to swell and become inflamed. Now I can hardly walk. Prior to all this, I was very active.

Two days after the vaccine, on Saturday morning (3/13/2021), I started to feel pain when standing and walking. The pain was in my shins and ankles. Throughout the weekend (3/13/2021 - 3/14/2021), the pain increased in severity, and my calves plus ankles began to swell severely. I remember weighing myself and being around 7 pounds heavier than usual. On Monday (3/15/2021), I was able to visit my Primary Care Physician's office for an ad-hoc/emergency appointment. Dr. diagnosed me with "bilateral feet swelling and ankle pain after he had his COVID vaccine". She asked and acknowledged that no other change in behavior or "other specific aggravating or alleviating factors" would have caused this other than the COVID vaccine. Dr. provided an "AMB REF to Orthopedic Surgeon", but they were unable to see me until Thursday (3/18/2021). Therefore, the Dr. said I should go to the Emergency Room in case of blood clotting or DVT. I followed her advice and went immediately to the suggested ER. I was admitted to the ER on Monday (3/15/2021) afternoon and remained there through that evening. They performed Doppler Ultrasound, Urine Analysis (UA), Complete Blood Count (CBC), and a Comprehensive Metabolic Panel (CMP), with "no significant findings". DVT and Blood Clotting were ruled out after the extensive tests conducted. The ER staff told me to elevate my legs as much as possible, and to wear compression socks up to my hips/thighs until the swelling subsided. The ER staff also stated similarly to the Dr., that they believed this was an adverse reaction to the COVID vaccine. On Thursday (3/18/2021), I was able to attend my appointment at the Orthopedic Clinic. They performed "B/L foot XR" (x-rays), with the result of "no fx noted, no degenerative changes, no other structural changes noted." Both the attending and the Medical Student assisting said it was likely due to the COVID vaccine. They encouraged me to continue elevating and wearing compression socks until swelling plus pain subsided. As suggested, I had a follow-up appointment with my Primary Care Physician's Office on Tuesday (4/6/2021) with the Dr. "reviewed ER records" and swelling was resolved by this date. Dr. "advised to continue with the 2nd dose of the COVID vaccine".

Swelling of left side of face and neck same side as vaccination. Severe ear pain/possible ear infections thought to also have lymphadenopathy.

20 hours after injection I experienced swelling of both hands as well as very itchy palms. This lasted 1 hour and subsided on it's own. No intervention.

Moderna vaccine dose #1 received in right shoulder on 12/31/20 at 2:15PM. Injection was uneventful other than sensation of pressure. I did notice at the time, but could not fully see, that the injection appeared to be much higher on the shoulder than normal. I immediately came home afterwards and relaxed. Approximately 2hrs later, I began experiencing extreme pain in my right shoulder. As the night progressed, the pain worsened to 10/10 with inability to move my arm. Later that night after requiring assistance to take off my shirt, I noticed that the bandaid overlying the injection site was very high, immediately below and bordering the acromion process. This was concerning but I was hopeful the pain would go away over the next few days. I took tylenol that night. I woke up multiple times during the night because the pain was so severe. When I woke up, the pain and inability to move my arm were still present. I began taking 800mg ibuprofen and 1000mg tylenol alternating Q4 throughout the next few days. The pain and disability remained so severe that I required assistance performing ADLs for the next 4 days. On day four, with the pain not resolved and still severe despite consistent advil and tylenol use, I began having concern for shoulder injury related to vaccine administration. The next day I decided to seek an evaluation by an orthopedist. I am a physician and was unable to perform basic tasks and ADLs. I happened to have a few days off after the injection but would not have been able to work had I not. After an evaluation by the an orthopedic PA, I obtained an MRI of my right shoulder with results shown below - as I expected evidence of rotator cuff tendinopathy and subdeltoid bursitis consistent with SIRVA. As of now, I still have limited range of motion and shoulder pain on the right which has improved slightly but is far from resolved. I still have difficulty with certain ADLs including putting clothes on and off, lifting items, and tasks that require raising my right arm above my head. I am concerned with the possibility of long term and/or permanent damage after reviewing the literature. I am also concerned about how many other healthcare workers the person who gave me my vaccine may be injuring and/or causing permanent harm to.

Rash/redness around injection site, swollen lymph nodes, fatigue. Going to urgent care on advice of vaccination staff

Rash with itchiness in the face, neck, arms, groin, shaft of the penis, scrotum, swelling of the upper and lower eyelid and face. Denied any neurologic symptoms, cardiovascular or respiratory symptoms. the itchiness and the rash is intense. the rash can be notice from distance. Consulted the primary care Dr. and was prescribed a course of prednisone of 10 day taper starting at 50 PO mg/day.

Low grade headache started at 19:30 Jan 6 and has continued through Jan 7. Tenderness at injection site, shoulder hurts to move and arm is sore down to elbow (feels bruised and weak). Nausea at between 17:30 & 20:15 Jan 6. Mild joint paint. Not all joints just the ones that normally hurt when I get sick. Overall feeling tired and low energy. Jan 7 - Jan 8: Redness at injection site and some minor swelling.

On the sixth day I started noticing an inflamation on my lymph nodes that is lasting still on my 9th day after my vaccine, then on the 8th day I started having big headaches with weakness and numbness on my body, I didn't develop fever but I had nausea and tiredness that are still present today 01/21/2021

All events occurred 1 day after vaccine: 1:00am- severe chills, slightly elevated body temperature (99.2 degrees F) 1:30pm - hives on right wrist (itching, red/puffy, and raised), lasted 30-45 minutes, treated with over-the-counter lotion; 10:00pm - hives covering entire body, (itching, red/puffy, and raised), lasted approximately 1 hour; No symptoms after.

Patient states, noticed lump on left side of neck (anterior cervical lymph node) swollen, and tender with deep palpation. Sudden onset upon waking up with no gradual increase or decrease in size or change of status. Did not get seen or evaluated until 11 days from initial onset, 14 days from date of vaccination.

We were having a drive up COVID-19 vaccine clinic. Due to egg allergy he was instructed to wait for 30 minutes. About 15 minutes into his monitoring period medical staff was radioed for assistance. When I arrived at his vehicle he was alert, oriented, and breathing normally. He complained of sudden onset of bilateral anterior cervical lymphadenopathy. His face and neck had erythema. He stated due to eczema he normally had some redness but this was worse. At 12:21 he was given Epipen to left thigh IM. Vital signs HR 84, RR 18 BP 120/80 SP02 99%. At 12:25 he was given diphenhydramine 25mg IM left delt. Vital signs HR 92, RR 16, BP 160/100 SP02 99%. As Epipen was given, another staff was calling 911. Emergency services arrived at 12:33 and took over care. Patient was transported by ambulance to the University location. When he left via ambulance he was in stable condition.

Patient come in to our office to get the first dose of COVID-19 vaccine on 01/15/21 . Allergies reviewed first , all documented properly . Patient reported that he wake up on the same vaccine date at 11:55 pm with pruritus in the trunk and back , he turn on the light and looking for a mirror to see what happen . He saw multiples hives in the trunk , back , inguinal area , internal area of the thigh , also he felt and saw swelling in the lips and trouble to swallow , denied difficulty to breath or chest pain .Patient immediately take 2 tablets of Cetirizine 10 mg orally and went to sleep, next day he felt better ....continued

I experienced pain and swelling in my right arm-pit, as well as pain in my right arm when I move it around beginning on 3/10

Patient said had the Moderna vaccine at pharmacy yesterday 3/24/21 about 2:30. Technician administered the vaccine with supervising in the same room. Patient came in today around 1 pm stating he was sore and couldn't sleep last night and today he has limited movement in left arm. He can lift it up to almost horizontal position forward but difficulty moving it backwards. He took 2 of the 200 mg Ibuprofen as directed and still sore. Advised he can take 4 of the 200 mg Ibuprofen (800 mg) every 8 hours for a day and see if that relieves pain and motion. He can also put a cold compress to affected area for slight swelling. If not better in a day or so advised him to contact a doctor, because might need something stronger to treat reaction to vaccine.

Within a minute patient became pale, cool and dripping sweat (pea size) from entire body. He stated "I don't feel well". He remained alert and oriented during the time period. Blood pressure 54/40 within 3 minutes; administered contents of Epi-pen 0.3 mg IM in left lateral thigh at 2:22pm and Benadryl 50mg IM in Right lateral thigh. At 2:27 blood pressure 104/54 Heart rate 89; His color was a little pinker and stop forming the balls of sweat. EMS arrived at 2:32 pm and took over. Patient express to EMS attendant that he did this once before when the lab tech had to draw blood. EMS took him to the local Hospital

pt said he was feeling anxious and sat near EMS, he was a bit sweaty and anxious. after sitting for 15minutes he felt better after getting snacks. pt decided to leave after feeling better and more calm. staff was with patient for reassurance and observation for any reaction.

time of injection, arm was painless, injection itself was painless. Noticeable swelling with no pain the night of injection. one day later (today) arm pain/soreness increasing and very painful. Fatigue set in 4/7/2021 morning after injection, and still feel fatigued 4/7 night (now at time of writing)

Aggressive red, swollen, dry, itchy, rash on hand of arm with site injection. Hive like puffy skin irration. Some have transformed into full blisters with secretion of clear/yellowish liquid. Using antihistamine and hydrocortisone cream and monitoring need to consult doctor if further worsens. In progress.

About a week (6 days) after I had my first Moderna shot, Inflammation of the hands and fingers started in the forms or red/skin color welts like Hives. Excessive burning and itching sensations, difficult even to wash or rub hands together, scratching was increasing the pain and was resulting in swelling of fingers. Fingertips had pain gotten super sensitive with inflammation. Couldn't even form my fist because of swelling and pain. Had a lot of discomfort and difficulty sleeping so took a Tylenol PM one night to reduce the inflammation and to aid the sleep. Only took it once as not much of a fan of medicines. I also soaked my hands in water with baking soda which provided some comfort and reduced the pain and inflammation, applied some coconut oil to soothe the inflammation. As I thought it was happening because of hives and/or excessive heat increase in the body, I changed my diets, started with foods and ingredients that cools down the body which felt like working and the swelling, pain, burning/itching sensations were slowly pacifying in 3-4 days. I also got acupuncture treatment after 5 days which has helped a lot now. Now after a week and all that I did, I don't feel the pain and burning sensations anymore, no swelling and the skin is getting clear, red spots/welts are disappearing and I am able to sleep better.

Noticed hives about an hour after getting the shot forming all around my body. Thought it was normal as the covid rash. Quickly started feeling ill. Had a small amount of trouble breathing and a fever with swelling and the hives became larger as my skin was flushed. Googled it and it said that this was a normal reaction and common side effects. So I did not do anything and fell asleep because I felt badly. The next day I woke up to see the bumps all over my body including my digits and pelvic region. Called and talked to my wife she told me to go to the hospital. I started feeling a bit better though and decided against it and took rest that day. The following day the rash had become raised and tender across my body.

Patient noticed tender bump in arm and next day and continued until about 14 days post vaccination. Not painful, but tender to pressure applied.

PT was given the Moderna vaccine around 12:15pm. Pt did not seem in any distress at time of injection. After injection, he moved to an observation area chair with armrests. He appeared to have a vaso-vagal response around 12:20pm causing him to pass out and fall out of chair. He hit his head on the floor causing a minor laceration on left eyebrow and some bleeding. He appeared to have some swelling around eyebrow and forehead area. His mother accompanied him and informed us post-injection that this has happened in the past, that he had syncope related events from vaccine. The paramedics were called and the PT was taken to the local hospital for further evaluation.

Patient presented to the ED on 5/3/21 with sharp chest pain since 5/1/21. He had received his second dose of Moderna COVID-19 vaccine on 4/15/21. The patient was diagnosed with gout approximately 1 week prior and had been placed on prednisone. The patient was found to have ST elevation MI with myocarditis along with cardiogenic shock. He was managed in the CCU requiring IV dobutamine and IV furosemide. Patient was transferred to secondary Medical Center on 5/4/21 for advanced heart failure therapy

Synopsis: 29 y.o. male w hx myocarditis of unknown etiology in September of this year presenting w six days of palpitations - a sx he associates w prior myocarditis. No CP or SOB. No dizziness. No weight gain. Had second COVID vaccine 5/12/21 and sxs began within a day or two following. Seen in rapid care and found to have tropinemia. Spoke w cardiology who requested an echo, motrin (confirmed w pharmacy), colchicine, and planned an echo. Reeval to follow - if trop downtrending, may be able to go. He would prefer discharge. 29yo patient with history of depression, OCD, who was admitted 8/2020 with chest pain for 2 weeks found to have episode of myopericarditis with troponin T peak 0.95 and Troponin I of 6.35. CRP was 14. At the time TTE showed LVEF 50%. He was treated with lisinopril and ibuprofen and had improvement of pain. He later followed with a cardiac MRI 9/2020 which showed LVEF 55% and patchy epicardial/midmyocardial enhancement consistent with myocarditis. Trace pleural effusion. Patient was called by his cardiolgist today. He reported he has been feeling unwell for past 5 days (similar symptoms to his first moderna shot) after his moderna shot #2. He had fatigue, body aches, chest pain, back pain and feelings of palpitations. He had some mild fevers and chills initially as well. These symptoms also occurred when he had moderna #1 (exact same). He was asked by cardiologist to come to the ED concerning for myocarditis given reports of myocarditis post vaccination. Upon arrival to the ED, patients vitals were stable. He does not have any chest pain at this time. He reports that he had some palpitations starting after his shot. He denies any SOB, nausea, vomiting at this time. His HR was 60s in the ED with sBP 120s/67 on room air. His CRP was normal 2.8 Troponin I POC 1.66 EKG without any significant changes from prior but slightly more accentuates TWI in leads II, AVR. 29 y.o. male w hx myocarditis of unknown etiology in September of this year presenting w six days of palpitations - a sx he associates w prior myocarditis. Concurrent medical conditions included Seasonal allergy. Concomitant products included CETIRIZINE HYDROCHLORIDE (ZYRTEC [CETIRIZINE HYDROCHLORIDE]) for Seasonal allergy. On 22-Apr-2021, the patient received first dose of mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) 1 dosage form. On 27-May-2021 at 5:00 PM, received second dose of mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) dosage was changed to 1 dosage form. On 28-May-2021, the patient experienced SYNCOPE (Fainted 3 times/can't stay more than 30 to 45 seconds) (seriousness criteria disability and medically significant), FATIGUE (Need to lay on the floor) (seriousness criterion disability), VACCINATION SITE PAIN (Injection site pain) (seriousness criterion disability), CHILLS (Chills) (seriousness criterion disability), PYREXIA (Fever) (seriousness criterion disability), MYALGIA (Body aches/muscle aches) (seriousness criterion disability), LYMPHADENOPATHY (Swollen lymph nodes) (seriousness criterion disability) and PAIN IN EXTREMITY (sore arm) (seriousness criterion disability). 28-May-2021, the patient experienced LETHARGY (Need to lay on the floor/not stay for more than 30-45 seconds) (seriousness criterion disability). The patient was treated with DEXTROMETHORPHAN HYDROBROMIDE, DOXYLAMINE SUCCINATE, EPHEDRINE SULFATE, ETHANOL, PARACETAMOL (NYQUIL) for Fever, at a dose of 0.5 dosage form. At the time of the report, SYNCOPE (Fainted 3 times/can't stay more than 30 to 45 seconds), LETHARGY (Need to lay on the floor/not stay for more than 30-45 seconds), FATIGUE (Need to lay on the floor), VACCINATION SITE PAIN (Injection site pain), CHILLS (Chills), PYREXIA (Fever), MYALGIA (Body aches/muscle aches), LYMPHADENOPATHY (Swollen lymph nodes) and PAIN IN EXTREMITY (sore arm) outcome was unknown. Starting the day after the vaccine, 06/04/2021, I had severe body aches, severe headache, pain on my arm where the shot was given, and severely swollen eyes but no vision problems. The body aches, headache, and arm pain lasted until 06/06/2021. The swollen eyes are still swollen today, 06/10/21, 7 days after the vaccine. On 06/09/2021 my lymph nodes in my right arm pit and on my left pectoral muscle swelled up, became red, and had an aching pain without touch and a moderate pain when touched. As of today, 06/10/2021, my lymph nodes are still swollen but with very minor pain with touch.

rash throughout body. rash is extremely itchy, located on face, bilateral arms, front and back of torso, bilateral thighs and feet including the soles. rash is asymmetric, with well defined borders, pink-red, greater than 1 cm in width/length. rash started on hands, then appeared on my feet, then arms, then torso, then face. on going for more than 2 weeks.

A couple of days after the vaccine, my lymph nodes in armpits were swollen and neck lymph nodes were swollen for a couple days. It was uncomfortable nut not painful. No fever, no malaise. I did feel nauseous during this time. 2 weeks after my vaccine (around 05/22/2021) I had a weird sensation that my left arm went numb and I couldn't really control it 100%. I went to the ER because I thought I was having a stroke and because of my experience with Bells Palsy. They didn't do anything because I was in the lobby waiting for 2-3 hours and they seemed rushed and they told me that I must've been sitting in weird position because my arm went numb. They didn't do any tests.

I got the 2nd dose on 06/06/2021 and then for a couple days after my heart was pounding and I was just tired. I felt tightening in my chest, intermittent pain in upper back. Periodically this would come in waves. My hands felt like pins and needles kind of flushed over my body. It was mainly hands and feet but I would feel it in my face and sometimes arms and legs. I did go to the ER about a week after 2nd dose (around 06/13/2021). They told me it could've been anxiety and stress. They did an EKG, blood work, and x-ray and didn't find anything abnormal. I did f/u with PCP and I just went last week and explained symptoms there as well and they're looking into my issues. They told me today I had a high white blood cell count and that my liver has elevated enzymes. They want me to come in the middle of July to do more fasting blood work. No fever, no malaise, but I did feel tired and fatigued and nauseous. I had bad brain fog and my body felt like it wasn't mine. It was like someone took my brain and put inside someone else's body. 1 week after 2nd dose I had bad sinus pressure behind eyes like my eyes were going to burst out of skull.

Per patient initially the first couple of days he experienced normal superficial discomfort in the arm and shoulder-but then on June 5th he began to feel the pain deeper in the arm. The pain continued to persist and according to him it was a level 10. He did go to the physician's office to get examined and per patient, MD stated it was bursitis and it would take time to heal. I did speak with him on 7/12/21 and he mentioned the pain is gradually getting better.

Left sided axillary lymphadenopathy, with self-limiting pain lasting approximately 3 days, and lymphadenopathy persisting.

About 11 days post vaccination I had noticed swelling of a left supraclavicular lymph node. The node was about the size of grape, tender to touch and caused some aching on left side of neck. Within 3 days the lymph node swelling started to subside however 11 days later, the lymph node is still swollen.

1/20/21 at 5:00pm injection site (right arm) became swollen and tender. Pain moved down to elbow and into hand. Pain caused difficulty sleeping. Awoke 1/21/21 5:00am and noticed pain moved into right armpit. Through morning pain moved into right side of chest, around lymph node area of under-arm. Swelling and tenderness persisted. At 1:00pm on 1/21/21 pain had not subsided which prompted concern and evaluation at Emergency Room. Ultrasound performed with no significant findings. As of 1/22/21 at 8:11am swelling and some tenderness persist.

Peripheral Edema, tingling nerves + numbness, swollen joints (Right Knee & ankle), pain behind the knee working down to the calf, swelling discomfort with or without shoes on.

fever >102 x 24hrs. 48hrs out still have fever >100. general aches and pains. left axilla lymph nodes very swollen, inflamed, and tender (cannot fully rest arm on side because of discomfort, injection received in left arm). stiff neck and fatigue.

Severe chills, trembling and shaking 12 hours after injection. Mild fever, sweating at night, inability to sleep, nightmares. Joint paint, heavy feeling in limbs, swollen supraclavicular lymph node on left side, localized (left side) and sharp feeling headache. Swollen feeling in eyes, discomfort when blinking eyes.

Chills starting around 10:30 PM, joined shortly after by an increasingly painful headache. Alternating between chills and being overheated on and off all night, while headache became worse. Had to take ibuprofen at 7:30 AM after not being able to sleep, and was able to sleep. Woke up at 1 PM and had to take another ibuprofen for the headache. Chills were gone, but was overheated and sweating for an hour or two. Had to take another ibuprofen at night, but headache wasn't too much worse. Headaches were gone upon waking up at 9 AM on Sunday. Experienced lymph node swelling in armpit of arm that got the vaccination. Tried to do pushups, but became too painful. I had also experienced the lymph node swelling after my first vaccination 28 days before my second one. About a week after, and they always felt minorly swollen up until the time of my second vaccination. I didn't know they were caused by the vaccine until after the second vaccination. Currently 1:20 AM on Monday, and I have been experiencing redness and itching in a large patch around the vaccination site all day.

30 yo man with no significant past medical history presents to the ED for chest pressure and SOB that started this morning when he woke up. It was a constant 8/10 pressure squeezing at the sides of his sternum with the sensation of a plate pushing down on his sternum, that worsened by taking deep breaths. He had never experienced anything like it before. The pain was neither worsened with exertion nor relieved by rest. It did not change with position, and it did not radiate to his arm/neck. He denies any tenderness to palpation of his chest. Ibuprofen 400 mg did not provide any relief. At time of interview denies SOB and reports decrease in chest tightness to 2/10. CP was significantly relieved with a bolus of normal saline in the ED. Tylenol also effectively reduced his pain. Of note, the patient received the second Moderna COVID vaccine shot 3 days ago. For 2 days after receiving the second shot he experienced fevers, chills, nausea, and intermittent vomiting (10 bouts of non-bloody emesis).

Anaphylaxis, severe body rash with hives that?s spread all over upper and lower body, along with neck and face.

Patient lives with family in private residence with parents and 2 siblings. Spends most of time in own room. First family member diagnosed with COVID-19 on 12/21 2020, the other 3 family members diagnosed with symptomatic mild COVID on 12/26/2020. Parents are healthcare providers and used specific informed diligent protective strategies to prevent infection in patient with minimal close contact and gloves, masks. Patient never had symptoms and was not tested for COVID.. Had first vaccine dose on 1/16. On 1/17 had shaking chills with temp to 101.3F, gone on 1/18.. No other overt reactions. Subsequently in usual state of health. On 2/15 he developed vomiting and fever with progressive abdominal pain and transient wheezing. Outpatient testing for SARS-CoV2 was negative and full respiratory pathogen panel (including SARS-CoV2) also negative. With worsening fever to 40.2C and abdominal pain, was admitted on 2/18. Found to have high leukocyte count, low lymphocyte count and transaminitis. CT imaging with terminal ileitis, thought possibly infectious but extended GI pathogen panel negative. Experienced persisting daily high grade fevers to up to 39.1.C. Developed transaminitis and hypotension and was transferred to intensive care unit on 2/21. Was intubated for hypoxia, agitation, required levophed, large volume resuscitation and empiric antimicrobials. Had transient AKI. Antimicrobials stopped on 2/22 with negative cultures. Colonoscopy performed with leal biopsy of identified ulcerative disease with focal active enteritis without specific etiology identified by stains. Ferritin rose to 24,000 with hypertriglyceridemia and thought to have possible HLH and underwent bone marrow biopsy which showed leukocytosis and lymphopenia, and rare hemophagocytosis. Dobutamine was started for low LVEF and levophed weaned. High dose dexamethasone started on 2/23 and pressor further reduced, dobutamine weaned to off on 2/25. Extubated on 2/24. Febrile still to 39 on 2/24 and with leukocytosis but transaminases, ferritin and triglycerides improving. Discussion ongoing about HLH vs macrophage activation syndrome. Currently meets 4 criteria for HLH with soluble IL-2R and NK activity pending.

Immediate onset of flushing sensation in face and upper extremities, along with slight swelling of the left upper extremity (especially in the hand). Benadryl 37.5 mg oral was administered, and symptoms gradually improved and resolved over the next 10-15 minutes. Patient did not meet the clinical diagnosis of anaphylaxis, and epinephrine was not administered while patient was in the office. Patient never developed respiratory compromise, GI symptoms, or reduced BP/end-organ dysfunction. Vital signs remained normal and stable following reaction and at the time of resolution of symptoms. It was recommended that patient go to the ED (by EMS) for monitoring purposes/delayed reaction. EMS transport was declined by patient.

He developed enlarged lymph node 2 days after vaccine in axilla region on the side of injection. Had low grade fever and chills for over a week and developed lymphadenitis. It worsened to the point he had to have Incision and Drainage 10 days after vaccine.

Pain under left arm (lymph nodes) developed 5 days after shot. Body rash (no rash on injection site) on elbows, hands, arms, hips, chest, back, feet, groin, and abdomen developed 6 days after shot - typically resides after a few hours, but recurs daily and has grown larger and in more diverse places over time.

Patient did not bring vaccine information for lot number and expiration date. Pt states he received in his right upper arm. States his "Face felt tingling" following administration of vaccine. Pt states he was monitored for 30 minutes at his VA clinic and sent home. He states he has a sore throat, nasal congestion, and lymph node enlargement with the left side of his neck. He denies rash, fever, shortness of breath, mouth or tongue swelling.

Left collar bone area is swelling up and causing discomfort. At first it was just the bone but the third day the whole top area of the collar bone felt bruised.

Experienced anaphylaxis. Similar to what I experience for peanuts. Symptoms: Difficulty swallowing, indigestion, high pulse

102 fever for 5 hours so far, chills, body aches (legs, arms), diarrhea, occasional severe nausea, occasional headache, minor swelling in left armpit, low grade constant pressure/pain on left and right side just above hips (kidneys, but front?). All "known" side effects so far, but I got all of them essentially simultaneously. I didn't think I needed to report if it's under 72 hours since injection since it is all "known" but my mother pressured me to file a report :) Spo2 levels 95-100, elevated heartrate (90-100), taste and smell are fine, no cough so far

Patient presented to ER severely hypothermic, and disoriented and what appeared to be sepsis with a leukocytosis. Workup was negative for any cause of infection and was even seen by infectious disease. Patient had received his first Moderna vaccine that morning, and had no prior exposure to Covid 19 virus.

Achiness, fatigue, and mild nausea on 4/7/21 (day of second vaccination). Chest pain, fever, achiness, fatigue, mild nausea on 4/8/21. Symptoms mostly subsided on 4/9, so I returned to work at 3:00 pm. By about 5:00 pm, the crest pain returned and worsened until I went home, and then to the hospital around 7:00 pm. Tests were done before I was diagnosed with "acute myocarditis, elevated troponin level, coagulopathy, NSTEMI (non-ST elevated myocardial infarction)", told I likely had a heart attack, and was put on a Heparin drip and kept for additional testing and observation until 4/11/21 at approximately 5:00 pm. Bells Palsy and Facial swelling in right side of face. Noticed a few days after receiving my first dose of the vaccine. Still experiencing symptoms and waiting to see a neurologist but hoping for a clear course of treatment

Patient reports sharp pain in temple within 2.5 hours of vaccine administration with feeling cold, having chills, and shaking. Patient has facial swelling on 4th day after vaccination and sent home from work with Benadryl, acetaminophen, and ibuprofen.

Sore lymph nodes in both armpits. Started in the left armpit on 3/8. Progressed to right armpit by 3/10. Soreness is mild, and is not always noticeable. Soreness has persisted, and remains as of today (5/14). Right armpit is more sore than the left. Lymph nodes do not feel very "swollen," but are sore to the touch in both armpits.

Fever of 102 degrees fahrenheit approximately 12 hours after injection. The fever lasted approximately 3 days, decreasing incrementally until it ended on day 4 Extremely run down, headache, fatigue, general fogginess. This fade away with the fever. Painless, swelling in both armpits, approximately the size of a lemon and felt fluid filled. The swelling slowly went away on its own after about a week.

Symptoms started approx. 24 hours later. Reports ?severe rash? along throat and chest, and swelling in face along cheekbones and under they eyes. Rash started to dissipate on its own after 1-2 days. Swelling continued so he started Benedryl on his own. Swelling decreased but has not completely subsided. Went to his PCP, she provided an Rx (wasn?t sure what it was at this time). Patient states PCP reports not worried about him getting a second dose

On the 15th I noticed a swollen supraclavicular lymph node (above the clavicle and at the base of the neck) on my left side. It is slightly tender to the touch and otherwise there's a general feeling of pressure at the site. As of today, the 23rd, it is still swollen and similar in size.

Moderna COVID 19 Vaccine EUA Fever 103, for 3 days, swollen left lymph nodes, nausea, vomiting, diarrhea, headache, chills, serve joint pain and pain at the injection site. All lasting for 4 days.

Moderna COVID-19 Vaccine. Headache and nausea onset 12 hours after vaccine was given. Chills/body aches and fever were present 18 hours after injection. Fever remained between 101.2 and 102.4, with a spike of 103.4 on 1/14/2021. Headache and nausea lasted approximately 24 hours, while fever and chills/body aches lasted for 5 days after vaccine was administered. An Emergency Room visit to Hospital was made on 1/15/2021 into 1/16/2021 for these symptoms since they had lasted over 48 hours. Negative on all tests, with elevated inflammation levels. X-ray and CT scan of chest revealed no pneumonia or blood clots.

I woke up the next day with a severe headache, body aches, chills, severe malaise, sore throat and my face had swelled at around my eye sockets close to the bridge of my nose. All these symptoms last about 5-7 days.

Debilitating fatigue and muscular pain could not move my arm at all along with numbness and tingling going throughout my right shoulder blade I also had severe swelling of my lymph node under my right armpit which was an agonizing pain that was unrelenting.

He also reports that he can?t fully open his right eye, which impairs his field of vision; otherwise, denies any vision changes. Instructed that the symptoms were suggestive of Bell?s Palsy, but to go straight to ER for further evaluation.

Temperature of 102 the next day after first shot, while also taking Motrin and Advil, nausea, intense shaking and body aches, foggy headed, swollen sensitive lymph nodes and body muscle pain on left side and low energy the first round of symptoms lasted about a week. Then I had severe migraines and throwing up even though I?ve never had migraines before. This lasted about a week. Then I began to develop painful lymph nodes and got shingles, also something that has never happened before. Shingles healing right as I got second shot on March 5th and had same high fever sick reaction as first week of second shot but with diarrhea this time which I did not have the first time.

Arm started to get itchy, was red and felt swollen, felt very hard to the touch. Probably the size of a baseball in terms of surface area. Lasted about four days. Today it seems to be gone. No redness, swelling or itching.

A few days after the vaccine my lymph nodes in my upper cervical area high in my neck area swollen which caused me to have a sore throat. A single small ulcerative lesion appeared in the upper soft palate of my mouth so I went to a walk in clinic and had a strep done which was negative. D/t the ulcers appearing in my mouth I had and HIV test done which also came back negative. I had a small low grade fever and muscle aches also associated but the lesion in my mouth is what caused the major concern which slowly resolved over the next few weeks.

The patient's medical history was not provided. Concomitant product use was not provided by the reporter. On 23 Feb 2021, prior to the onset of the symptoms, the patient received the first of two planned doses of mRNA-1273 (lot number 022M20A) intramuscularly for prophylaxis of COVID-19 infection. On unknown date, the patient developed abnormal feelings on his lateral right upper ileus, his chin bone, tangling, facial dropping right upper chin, mild welling to his right eye and chin. He can't open his right eye. Treatment information was not provided. Action taken with mRNA-1273 in response to the events was not provided. swollen mass in left axila. started feeling anaphylaxis-like symptoms, including swollen throat, face/jaw numbness, inner ear swelling. the symptoms appear random with varying intensities, no apparent trigger

On 4/12/21, nearly a week after my first dose of the vaccine, I experienced intense, sharp pains in the left side of my neck. The pain was intermittent throughout the day and didn't seem to be triggered or alleviated by any specific action, It coincided with a slightly enlarged, hard lymph node along my left external jugular vein, about two inches below my ear lobe. The pain persisted (intermittently) lasting no longer than one to three seconds each time, or in some cases pulsing on and off for 30 seconds. In total, I counted roughly a dozen of these episodes between 4/12 and 4/13. The lymph node has since decreased in size and no longer hurts.

Symptoms/side effects began about twelve hours after second dose vaccine injection. Symptoms began weak to mild and increased to severe over the next twelve hour period, then subsided with no apparent lingering affects. Symptoms and side effects included body chills - becoming severe, mild body aches, strong headaches, very vivid hallucinogenic dreams that included strong physical stimulus, poor sleep and awoke very confused and disoriented, and strong fatigue. Weak soreness around vaccination site, notable less so than first vaccine dose, and no noticeable underarm lymph node swelling like first dose.

Fever, chills, injection site soreness, swollen lymph nodes in neck, body aches, headache, and dizzyness. Went away after 12 hours.

Patient said his lymph nodes were swollen and on day 8 a entire layer of his hands peeled off. Patient presented on 5/6/21 still with peeling on his hands.

My son his paralyzed t12 and his left hip swelled up within 24 hours of injection, then he used a hand massager to loosen it up and the leg and knee became bright red, swollen even more, and the pain in the leg became unbearable he couldn't move without intense pain, he was taken to ER, where he was treated for possible infection, and they noticed his hemoglobin was dropping and his leg was getting larger, he received 3 units of blood during his stay, A hematoma developed and was growing in the left thigh. He was given Vancomycin, then Doxy. The blood stabilized day 6 and he was discharged day 7 to home to continued recovery.

Patient received her first dose of Moderna on May 6th in the late afternoon called our office on May 10th explained that she developed headache and body aches the evening of May 6th and woke up on May 7th with pain/swelling on the left posterior head and then noticed a painful lesion on her scalp. Dr advised patient to come in for evaluation.

Starting on May 20th or 21st I noticed a pain while putting deodorant on under my left armpit. As I went through the day, I noticed that it started to swell under my arm. As the days went on it got continually worse. On Sunday night, in hopes to get the swelling down used Ice & Heat to try and combat the swelling. It did not work. Today 5/25 the swelling is still there along w pain.

Patient is a very pleasant 31-year-old white male who previously healthy, he works with a maintenance crew, he was in his usual state of health last week, he did receive his 2nd dose of the COVID-19 Moderna vaccine on Thursday. Over the next 24-48 hours he began experiencing myalgias malaise and fatigue, he self treated with ibuprofen and Tylenol as needed. He felt a little bit better on Sunday and went to assist in picking up a brush more. During the process he became short of breath as well as midsternal chest discomfort. He presented to the ED. Evaluation found a CRP of 112, elevated troponins and a leukocytosis of approximately 20,000 along with elevated neutrophils. His influenza a and influenza B testing have been negative and his SARS COVID the PCR was negative. Given his elevated troponins, he was started on IV heparin and admitted for further evaluation. On admission to the floor, he was chest pain free. We were concerned for potential myo/pericarditis given his history and laboratory study findings. Cardiac MRI was performed which suggested active myocarditis with biventricular systolic dysfunction (LVEF 34%, RVEF 37%). Given his new LV dysfunction he was started on low dose lisinopril and metoprolol succinate. It is unclear whether this is atypical viral myocarditis or whether it potentially could be vaccine related, the MRI is consistent with that of myocarditis. We did not proceed with cardiac biopsy unless since there was no evidence for clinical decompensation, unstable heart rhythm or other worrisome features. He will plan to follow up with Dr. 1 week post hospitalization with repeat echocardiography and blood work. If at that time there are any signs of instability we would proceed with more aggressive workup at that time. Infectious disease was consulted who recommended infectious work up including serology testing for: parvovirus IgG and IgM was positive, Herpes Virus type 6, EBV, CMV (undetected) adenovirus, enterovirus (negative) and HIV (negative) . He does not have any family history of cardiomyopathy. There is no family history of sudden cardiac death, he states that his family has been quite healthy from a cardiac standpoint he was previously healthy before this episode.

Pt w/smoking hx and strong family hx of premature CAD p/w worsening severe, dull ache pain radiating in left arm that's been intermittent over past week and associated w/tightness in neck/chest & diaphoresis/SOB. Did receive 2nd Moderna vaccine prior week (5/7 & 6/4) and described feeling poorly x24hr, then started feeling above symptoms. EKGs showed sinus rhythm w/ST elevations (V3-V6), II, & aVF w/troponin 8.32 and 3-hr troponin 13.58. Was admitted & started on hep & nitro gtt, ASA, beta-blocker/ACE-I. CRP=6.1, ESR= 17. CXR was clear. MRI confirmed myocarditis & heart cath revealed no obstructive CAD. Pt improved & was ultimately discharged w/weekly CRP & clinic f/u in 1 mos.

First Dose: I took Moderna COVID19 vaccine and passed out immediately onsite. One of the nurse have to call in ambulance services , they checked ECG and transported me to nearby hospital's emergency department. Where I received 3-4 hours of emergency treatment ( a bed, re-do ECG, an emergency doctor's visit and Benadryl I.V. injection). All this life threatening events occured to me because of Moderna COVID19 vaccine. I was very excited and walk on my feet to clinic to take vaccine but I have to go in ambulance and face all this difficulties at the cost of my job day off and all other related inconvenience. Both ambulatory services and hospital sent a huge bills, which upon my request, they submitted to my health insurance plan. Insurance company paid some amount but not in full. Insurance company said there is no government mandate for insurance companies to pay COVID-19 vaccine related emergiencies. so, they are not liable. Second Dose: Despite all this challenges, I decided to go for second dose to protect myself and the people of the from COVID-19. Again the same above story repeated even during the second dose. and they send bills to me. Now there are close to $3000 bills on me due to above circumstances, which I don't think appropriate for me to pay. I was simply aiming for safety for myself and other general public by getting myself vaccinated without any prior knowledge of this could cost me financially and dangerous to my own health. I was simply following CDC guideline and suggestion. In this situation, could you help me financial assistant or request ambulatory services and health to waive my financial obligations? Also, I request you to ask Moderna to reimburse for the cost of adverse effects of their vaccines.

Received my vaccine on Wednesday 12/30/20. The following Tuesday 1/5/21, I woke up with uncomfortable swollen Axillary lymph nodes. Thursday evening (1/7/21) I noticed redness and slight swelling at the injection site. Swollen nodes continue.

A few hours after the injection, my left arm started to hurt more than expected and decreased my arm mobility for the day. When I try to use it, it caused mild pain. I received my 2nd dose of the Moderna Vaccine on Wednesday the 27th of January. Immediately I felt soreness in my arm. About three hours later I began to feel light headed and fatigued. Over night between the 27th and 28th I became feverish, with chills, shaking, and sweating. On the 28th I experienced fatigue, lymph node swelling in my neck and left armpit, and all over body aches. On the 29th I continued to experience fatigue and pain in my lymph nodes. In addition, I began to experience a migraine headache which persisted for about 30 hours. My symptoms generally abated by the morning of the 31st .

Severe hives and swelling in face/lips appx 3.5 days after getting 1st dose of Covid-19 Moderna Vaccine. Blood test showed that I am suddenly highly allergic to 19 different food groups. I have no known food allergies prior to C19 Moderna Vaccine. I have had hives every single day since they 1st appeared (3.5 days after vaccine). I have had multiple days of facial swelling as well and have been to the hospital multiple times. To this date (date of this report) I still have hives and facial swelling. Facial swelling is intermittent. I have stayed away from the list of foods that I am apparently suddenly allergic to and I still have hives and swelling. I occasionally have a irritated throat which makes it difficult to breathe in. I have a specialist appointment with an allergy/immunology doctor this coming Thursday to see what thier diagnosis is. My family doctor believes my potential angioedema diagnosis is directly related to my 1st dose of C19 Moderna Vaccine.

Adverse event: Mild fever, body aches and fatigue 24 hrs after 2nd dose. Treatment: Ibuprofen and acetaminophen every 4 hrs . Outcome: reduced signs and symptoms. Extreme flu symptoms including headache, vomiting, fever, muscle fatigue, aches, chills, and soreness. Then I developed swelling in the arm pit on the arm I was injected.

Headache and mild foot pain started 19 hours after vaccine given. Patient went on a run about 72 hours after vaccine given and developed facial swelling, wheezing and severe bilateral foot pain and swelling.

Pain in left armpit; slight swelling and tender to touch. First started approximately one week from injection date; lasted four days. Has currently subsided.

Quarter sized hives on elbows, arm pits, and the side of torso. My hands also felt swollen and hurt to use. Symptoms appeared 20 hours after shot.

The actual vaccine site no longer has any tenderness, and it was never painful after the shot. I had no symptoms at all between 3/22 and 3/28. The lymph node under my right arm is visibly swollen as well. On 3/30, I was given the results of my covid test from 3/29, which was "covid-19 not present."

Pain on the right foot Foot began to swell up Two large bumps emerged Excruciating pain for 20 minutes - thought I was going to die and I was going to lose my foot Lied down, put pressure off the foot, kept stretching and massaging. After 20 minutes all pain went away. Little numbness. Next day completely recovered.

Patient developed pain radiating up into the right side of his scalp starting about two weeks after the injection which was in the right deltoid. . The pain then progressed to the right neck and supraclavicular area. He felt throbbing , sore, numbness as well and "shocks". Over the next view days the pain was shooting down his triceps and numbness and tingling and then numbness in the fourth and fifth fingers. No muscle weakness and no fever or chills. No systemic symptoms. He complained of the tingling numbness and a sense of swelling--all down the right arm in the c6-7 distribution. On April first his wife noted a vesicular rash on the right side of his neck with a few lesions on the right upper back. He was not responding to NSAIDs, ice or heat, immobilization, so at this point the clinical diagnosis of shingles/varicella zoster was made and Valtrex 1000 mg tid was started.

Shingles - Diagnosed 40 days after second vaccination (05APR2021). Symptoms developed 37 days after second vaccination (02APR2021). Painful, swollen lymph nodes at top of groin; red blistering rash, initially very mild, later very inflamed. Shingles diagnosis made at health clinic.

The patient experienced an anaphylactic reaction characterized by throat closing, shortness of breath, cough, and inability to speak due to airway narrowing. The symptoms started at 0800 and were severe enough that he was having difficulty speaking, so he had a friend talk to dispatch. He was given 2 doses of IM epinephrine, 50 mg Benadryl, and 125 mg Solumedrol by EMS and transferred to the ER department at the Community Hospital.

Muscle pain, headache and hard time focusing on daily activities appeared 7 hours after that shot and didn't go away for nearly 24 hours. On April 17th, two days after the shot I discovered a large bump (around 4-6 mm) under my skin on my lower neck, on the left side, close to the shoulder. I called a doctor and was advised to observe the bump for a week and schedule an in-person appointment with my doctor if the bump doesn't go away. The doctor said that it's most likely a lymph node.

I got my 2nd dose in the left arm,. About 35 hours later the lymph node on the left side of my collar bone has become painfully inflamed. Then the lymph node on the inside of my left arm has also become painfully inflamed. The injection site of my first shot on the opposite arm has also become sore.

Upper arm felt swollen about a week after injection, rash began to appear and area felt hot starting 5/3. Rash still visible but is reducing when icing the area.

The shot was given up by my shoulder. About 3 days later, I noticed my arm had swollen from my shoulder to down a little pasted my elbow to my forearm. I have noticed the swelling is slowing in spreading down my arm. It is currently about halfway down my forearm.

05/05/21-around 9 hours after vaccination developed chest pressure and mild shortness of breath. Symptoms worsened over the next 2 days and on 5/7/21 pain intensified. Pain is described as chest pressure, shoulder and back pain, and also endorsed shortness of breath due to tightness and constriction. 05/07/21-presented to ED in the morning and was admitted to the Coronary Care Unit, treated with colchicine, NSAIDs resulting in gradual improvement of his symptoms. Given response to treatment and EKG changes the presumptive diagnosis of pericarditis was confirmed.

5/8 to 5/9: sore arm, low-grade fever, chills, muscle aches, general malaise, diarrhea. 5/10: muscle aches, including chest pains, severe headache, numbness and tingling in left arm, diarrhea, nausea, vomiting, dizziness, malaise. Thought I was having a heart attack at first. 5/12 to current: occasional muscle aches, headache, fatigue, diarrhea, "tightness" in the chest. Swollen lymph node next to left side of clavicle since first Moderna dose 4/9; GP administering second dose joked I should wait before getting my next mammogram.

Patient called to report that his arm had swollen, he also experienced back pain, and vomiting the morning following the vaccination. He went to the nurse at his workplace, she advised him to take benadryl and tylenol, which has helped.

Moderna EUA - Patient who is nonverbal and received his second COVID vaccine today reportedly developed evidence of discomfort by closing his eyes, gritting his teeth and trying to lay down within 15 minutes of his injection. This had not occurred w/ his first injection on 4/19/2021. A medical assistant reported seeing distended right neck and right arm veins, and his family member reported seeing urticaria. Vitals per MA were bp 97/59, pulse 102, pulse ox 96% on room air. Upon my arrival, there were no visible distended neck or arm veins, and patient was seated comfortably, upright in the backseat of his family member's vehicle. There were horizontal linear indentations across his right neck, as though he had been resting his head against the back of his seat, or a seat belt may have been rubbing on it (but seat belt would have been on wrong side). He was in no respiratory distress. There were no visible urticaria. BP was 115/69, pulse 92. Lungs were clear to auscultation and cardiovascular exam revealed mild tachycardia, regular rhythm, no murmurs, rubs or gallops. He had received 10 mg of Benadryl through his G tube just as I arrived. I recommended that patient be observed for 15 more minutes and patient went home without further incident, to go to ER if developed respiratory distress.

Pt stated that he was having an allergic reaction to the vaccine. He appeared with swollen and red face ( eyes and ears). He stated he has NKDA and had no reaction to first dose. He also stated that he didn't know if it was due to vaccine or bug bite. We gave him two Benadryl 25mg tablets and had him sit in waiting room for observation. After 15 minutes, he stated that he felt better , would continue Benadryl and contact doctor if symptoms worsened. I followed up the following day, he stated that reaction resolved completely within five hours.

Chest pain, malaise 1 day after 2nd dose, day2 after dose fewer chest pain episodes, day 3 intense chest pain prompting ER visit

Fatigue dizziness on site. Month later constant lymph node swelling feels like needles, abdominal swelling/constipation, abdominal pain. No other health issues at the time or prior to getting vaccination

Tiny red blisters which grew and became itchy and painful. Took 6 weeks to resolve. Appeared again couple wks ago. More than before. They hurt and itch more than before.

In addition to "normal" symptoms, (fever, aching, sever headache, fatigue, body aches, bloating, water wight gain) racing heart, hard heart beats. Slept 20 hours on 05/16/2021. Sever chest pain. Experienced jaw pain also. Chest pain has persisted. Much worse when laying down. Gets better when sitting up. As of today, still experiencing significant chest pain. No chest pain previously.

3 Days after shot tightness in chest occurred, on the 4th morning patient went to Hospital and was admitted with myocarditis and pericarditis following x-rays, ekg, and bloodtests showing elevated troponin and white blood cell count. Next 24hrs patient was monitored, given Indomethacin, and lovenox. Patient responded well with no continued chest tightness and was discharged 5 days after initial vaccine.

32yo M, presents to clinic after episode of syncope, and on presentation to clinic c/o SOB/DOE SpO2: 88-90%, left calf swelling, EKG showing S1Q3T3 signs and symptoms concerning for PE. Pt denies any prior history of blood clots and no risk factors on initial presentation. Pt reports only thing new has been receiving Moderna COVID-19 vaccine on 16 Jun 2021. He was transferred immediately via ambulance to hospital for high level of care, where PE was confirmed w/ Chest CT, and also noted to have markedly elevated D-dimer and cardiac enzymes. Pt was then reportedly transferred to hospital for definitive tx.

Diagnosed with minimal change disease- nephrotic syndrome on 6/24/21. Received Moderna vaccine on 5/18/21 and 6/15/21. Had minor symptoms related to minimal change disease in early May but not significant enough to warrant medical evaluation. Increase BLE swelling starting 6/9/21 and became progressively worse with associated weight gain of almost 20 pounds in two weeks after onset of symptoms on 6/9. Oliguria, proteinuria, ascites, abdominal distention.

Patient was vaccinated in upper left arm with COVID Moderna vaccine on 12/30/20 at around 4:25 p.m. Around 5:00 p.m. patient developed tingling feeling in left side of face which lasted remainder of day. Patient took Benadryl which seemed to help symptoms. On 12/31/20 left side of face had slight swelling which went away over course of day. Tingling has continued on and off since 12/30/20 and at times has manifested as mild jaw or muscle pain on left side of face. No diminished function in eye, mouth, and no paralysis or sagging in facial features.

After about 15 min: hr went to over 170bpm , flushed sensation, brain fog, was driving at the time and my brain wouldn?t figure out how to call 911 , eventually I figured it out but it took extreme mental effort. Paramedics came and my pulse was From 100 - 170 changing rapidly, bp 150-175/x changing rapidly, symptoms would come and go about 8 times on way to hospital. In ER same thing then about 2 hrs go by and then all started again. Pulse remained at 90- 100bpm at least until 2am. (My resting hr is between 40 and 50, my normal bp is usually sub 120/80) I later realized my hands were swollen, mostly on my left side. Lingering symptoms include brain fog, tiredness, and headache. Maybe an exasperating issue was I had low potassium levels likely making symptoms worse - I worked out heavily early in the morning, probably had low intake of potassium that day and day before, and maybe slightly dehydrated from my workout - I do Functional lifting, run, and other types of exercise regularly.

I had a little swelling at injection site on day of and night of. The following day the swelling increased and had spread to the armpit area. It was more tender and there was moderate to sever pain in the armpit. I took some advil with minimal relief. On the 3rd day (today) the swelling seemed a littlw worse and the pain increased slightly. I contacted my plastic surgeon (since back in October 2019) because i had a procedure called miradry, which elemintes the sweat glands in my bilateral armpits. The doctor advised me to come in immeidately to the office. Upon arrival the doctor examined the area and was able to se immiedalty that is was swollen. I consented to pictures being taken. He is making an entry as well as I am and I was advised to ice the area and to take advil and to remain in contact.

Covid-19 vaccine EUA; 7 days after injection left sided axillary and sub clavicular lymph nodes became swollen and painful; 9 days after injection the site of the injection became red, warm, rigid, painful.

Day 1 was dizziness and haziness, then pain and exhaustion. Night 1 was fever, shakes, chills, uncontrollable bowels. Day 2 was a POUNDING headache in my eyes, sides of my head, swollen lymph nodes, and on my jaw. Night 2 was sweats and nausea. Day 3 was coughing and mucus as well as muscle pain.

Arm hurt through this past Tuesday. Woke up this morning with shoulder/arm very swollen and painful at injection site.

Localized right arm tenderness, very minor headache, very minor body aches, right clavicular lymph node swelling/tenderness.

Patient states 8days after started to have swelling, little redness, warmth. States redness grew, more muscle pain, more swelling, darker red in color, generalized arm swelling and tingling in hands. No systemic symptoms: fever, chills, nausea, vomiting, diarrhea, shortness of breath: just localized to that area. - drainage, redness, warmth to touch. Start taking zyrtec twice daily for any allergic reaction. No need for steroids- swelling improving, declines any other systemic symptoms. Discussed topical- not interested. Advised if having worsening- red, hot to touch, fever, chills, drainage from site, streaking redness let us know and will start on antibiotics- declines at this time.

Patient called 02/08/2021 at 0800 and reported a severe rash on his right lower/mid back, appeared within the first 24 hrs. post admin of 1st dose which was at 11:00 AM on 02/05/2021. Patient stated he noticed that rash began to appear around 0900 on 02/06/2021. described rash as small ?pimple? like bumps, raised, itchy, x 2+ days. Patient forwarded a picture of the rash. Patient states he took no medications (oral or topical) to relieve symptoms. Prior to ending phone call at 0825 on 02/08/2021, patient states rash remains with raised pimple like bumps and that rash seems to be contained to same initial area of right lower mid back. Stated rash continues to be slightly irritating and slightly itchy. Does not feel that he needs medical attention at this time.

Lymph-nodes on left side of body became inflamed, spleen became enlarged causing severe discomfort in thoracic cavity and abdomen. Fever, Intense Headache, blurred vision, impaired judgement and vomiting. Course of treatment bed rest for two weeks with hydration and nutrition focused on spleen health.

High fever for 1 day (105-107). For the following week, swollen lymph nodes, loss of voice, spiking fevers at night, night sweats, chills, body pain (localized to hips and below, ringing in ears, and insomnia. Some symptoms still ongoing.

Onset of facial swelling and hives, including around lips, but no throat or tongue involvement, about 30 hours after vaccination. Worsened and spread to neck within 80 minutes of onset. Mild chest tightness. No shortness of breath.

Swelling on the right side of my body. My lymph nodes in my armpit were painfully swollen and now my right armpit right peck, right side and right shoulder and bicep are all swollen. The lymph node swelling has subsided a little after 4 days and is no longer painful but the ancillary swelling has not gone down.

Red rashy/swollen area in the pit of my left arm, which is the same side as the injection. It started on Sunday, 3.7.2021 and is getting worse.

2 days after receiving my first dose my tongue began to feel tingly and swell, I could feel the tightness in the back and in my lymph nodes as well. I began to have sensitivity to spicy foods on Thursday. On Friday I woke up and my words were slurring as my tongue felt more swollen and I started to notice some slight swelling in my throat, maybe 5%. I started gargling diphenhydramine on Friday night and that helped a lot, I also took loratadine and Fexofenadine. By Sunday my symptoms were masked by the allergy drugs and I stopped taking the diphenhydramine. 1 week later, 12 days post Moderna Vaccinne I stopped taking the fexofenadine and Loratadine. I continued to have tongue irritation and a sore on my tongue for 2.5 weeks. I then had an adverse event after the 2nd shot as well.

This is second symptom I've had with this vaccine the first was cold chills an entire day That happened on 03/17/2021. The reason I'm filing this is for today I started having slight pain in my left side lymph node in neck and then pain in my jaw both sides.

The patient received his first dose of Covid vaccination on Saturday 3/27/2021 (at 1:48pm) and called the pharmacy back around 4:30 to report swelling and numbness on the right side of face. The pharmacist on duty instructed him to go to the nearest emergency room.

Whole-body edema, particularly pronounced in feet and hands. Onset: 3 days after vaccination. Got worse over 5 days, then gradually improved.

I noticed swelling on my underarm on the right side beginning mid day yesterday (2021-04-07). Swelling was not noticeable at first, it was first noticed because of tenderness which continues to today (2021-04-08). Swelling became noticeable overnight or this morning. Only slightly tender to direct touch (was initially not at all), most of tenderness is felt from moving arm back to position at side. No other symptoms. Sent message to PCP today (2021-04-08) about symptoms but have not heard back. Currently just observing

3 days after vaccination side effects occurred. pain in shoulder, sore elbow, numb thumb, sometimes numb pointer finger, swollen left lymph node, loss of strength in shoulder, should feels strained. have taken ibuprofen and rested arm for 2 weeks. it has improved but numbness in hand and shoulder pain and loss of strength are still present

My husband got a headache and a rash on his temple, starting the Wednesday after his second shot. His headache and the rash got worse, along with swollen lymph nosed on his neck and head. He went to an urgent care clinic on Saturday, a week after his second Moderna shot, and was diagnosed with shingles. He was given gabapentin, a generic of Valtrex and prednisone.

03/28: At 9 PM (9 hours after shot), I rapidly experienced fatigue, fever (101 degrees), chills, aches, and slight nausea. I took my vitals during this time: SPo2 of 98-99%, BP was 123/80, and heart rate was in the mid-to-high 80s. By midnight, I felt much better and assumed everything was over. 03/29: Woke up feeling fairly tired. As I was trying to do my normal morning routine of cleaning up, taking out trash, etc. I noticed a shortness of breath. No fever or other symptoms from the night before. I checked my SPo2 levels with 3 different oximeters, and all gave readings in the 93%-94% range. My heart rate, which is usually in the 80s and 90s when moving about, was in the low 70s, bottoming at 72. This was nerve racking. This along with intermittent dizziness persisted for 24 hours. I got COVID tested and it came back negative. Flu came back negative, too. A consult with a doctor inferred that "everyone has different vaccine side effects." Not helpful. This shortness of breath/fatigue/low blood oxygen saturation continued persistently for the next 48 hours. I was essentially advised to stay home and rest. I noticed some lymph node swelling, too. By day 4, I was feeling much better. My heart rate had gone back up into the 80s, and my blood oxygen sat. had increased to 96-97%. I was still somewhat tired and got winded very, very easily. It's been over two weeks since my second shot upon writing this. I didn't want to report, but I can't make heads or tails as to why this happened. The only change in my health was taking the vaccine. I don't have any blood clots, asthma, or underlying issues. However, these side effects, if they are as such, need to be discussed. I know others who got the Moderna vaccine with similar anecdotes. The difference is, living with a clinician, I monitor my vitals all the time and knew enough to take measurements.

Swollen neck lymph node on the left side (same side as injection arm). Symptoms lasted approximately 48 hours.

Patient reported that his left arm was feeling numb and he felt like the fingers on his left hand were slightly swollen. He reported to the vaccinator these symptoms approximately 10 minutes after his vaccine was administered. He was given an ice pack and diphenhydramine and asked to extend his wait time for further monitoring. at 30 minutes he reported his symptoms were improved.

Left arm was swollen and VERY sore. Symptoms lasted until this morning. Also Got the chills when I was trying to go to sleep and very vivid dreams.

Much pain in left arm, swollen as well, within 12hrs of shot. And high fever after 15-16hrs of shot, highest temp. was 103.7F. Took Tylenol for the pain and fever. The arm pain and intermittent fever was for total 24hrs.

Patient woke up on March 11 with vomiting and diarrhea, continued feeling ill through to the morning of March 15, when he was found to be lethargic and unable to communicate. I took him to urgent care where he was immediately rushed to the hospital emergency room at local hospital. He went into cardiac arrest for 20 minutes while in the ER. Besides the cardiac arrest patient presented with the following: Acidosis, acute renal failure, acute respiratory failure, shock, severe sepsis, septic shock, sepsis, right ventricular enlargement, pleural effusion bilateral, pericardial effusion, etc... Patient was in ICU and on a ventilator for 1 week, then 5 days in telemetry. He was released when stable as a diagnosis could could not determined. Discharge instructions were to follow up with primary care Dr for additional tests. Patient was discharged on March 26, 2021. He went to hospital March 30 for blood work for a primary care appt on April 2. Patient's blood pressure was again very low and his sodium level was dangerously low, he was again admitted to the hospital. After exhaustive tests it was determine he has Addison's autoimmune and Hashimoto's autoimmune diseases. He was discharged from hospital on March 5, 2021. Patient was put on hydrocortisone, fludocortisone, and levoxothorine. He is now doing fine on the medication.

On 28-Apr-2021, the patient received first dose of mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) 1 dosage form. On 29-Apr-2021, the patient experienced PYREXIA (Felt feverish/fever of 103.8) (seriousness criterion hospitalization), JOINT SWELLING (Joints were swelling), DIZZINESS (Felt dizzy) and ARTHRALGIA (Sore in the knees and joints). On 30-Apr-2021, the patient experienced GAIT INABILITY (Unable to walk). On 03-May-2021, the patient experienced SEPSIS (Diagnosed with sepsis) (seriousness criteria hospitalization and medically significant) and ARTHRITIS (Diagnosed with acute arthritic response) (seriousness criterion hospitalization). The patient was hospitalized on 03-May-2021 due to ARTHRITIS and SEPSIS, and then for 3 days due to PYREXIA. First injection I had no adverse effects. 10 days after my second injection, the lymph node under my arm -- the one that received the injection -- flared up again (it had become inflamed the first three days after the second injection but quickly got better). It's been about five days, and it seems to have gone down / that pain has largely subsided. Additionally, both upper arms feel "heavy" and my hands are weak. It's not as easy to make a fist. It's hard to describe, but my arms just feel a little weaker.

At about 4 days of fever up tp 102, tons of sweating, chills, I would sweat so much through my bed linen. I had general malaise. Day 3 or 4, I started having nausea and vomiting, I was taking ibuprofen for fever and Zofran for nausea. I was taking Emergen-C . At night I was taking Nyquil to help me sleep. After fever let up I still had body aches, I went got a COVID test, I started having swelling in my lymph nodes in my throat. My toes started getting white, my toes went numb the only way I can describe it as Raynaud's phenomenon.

Patient received vaccine appx. 11am, went golfing after work, appx. 4:30pm. Began feeling "crummy" appx. 8:30pm, feeling "achy" and somewhat "sick with no appetite". Went home, began feeling more achy with chills. Ongoing sweating, chills, aching throughout night. Appx. 2am reports feeling "disoriented/confused", thought he was "hearing noises (unable to clearly describe these)". Reports feeling "crummy" after first vaccine, but nothing similar to this. Spent following day in bed, not feeling much better. Took Tylenol PM 650mg on following night; next morning reported feeling somewhat improved, but still under the weather with new notice of visible lymphadenopathy of left axillary nodes; describes as golf-ball sized and tender.

Shingles on the right side of my face. Symptoms include cold sores, mouth sores, severe tooth pain, some pain in my eye, no serious issues with the nerves of my right eye. Put on Valtrex. As of today, 1/15 still have symptoms but am improving.

The patient's medical history was not provided. No relevant concomitant medications were reported. On 30 Dec 2020, approximately 6 days prior to the onset of the events, the patient received their first of two planned doses of mRNA-1273 (026L20A) intramuscularly in the left arm for prophylaxis of COVID-19 infection. On 05 Jan 2021, the patient reported that he experienced swelling and stiffness around his collar bone, a pea-sized lump, pain on his left side and face and chin numbness for 2 days. Most symptoms resided at time of report, but the patient was still black and blue on the left side and tender at his collar bone and shoulder. Treatment for the event included heating pad and paracetamol. Action taken with mRNA-1273 in response to the events was not reported. The event, face and chin numbness, was considered recovered/resolved on 07 Jan 2021. The events, swelling, stiffness around the collar bone, pain on the left side, a pea-sized lump, was considered resolved on an unknown date.The events, black and blue skin on the left side and tenderness at the collar bone and shoulder, were considered not recovered/not resolved.; Reporter's Comments: This case concerns a 34-year old, male patient, who experienced a serious unexpected event of numb chin syndrome, and non-serious unexpected event of musculoskeletal stiffness, pain, swelling, nodule, contusion, and tenderness. The events occurred 7 days after first dose of mRNA-1273, lot # 026L20A. Treatment included Tylenol and heating pad. Very limited information regarding this event has been provided at this time. Based on the current available information and temporal association between the use of the product and onset of the events, a causal relationship cannot be excluded.

Golf ball sized mass swelling, hot to the touch. Painful in movement and palpation. Extreme fatigue for 36 hours. By 72 hours after the shot the swelling has shrunk in height but spread out to about 4" wide circumference. Still hot to the touch but less painful on movement and palpation. A lot less

The day after my vaccination, on 2/4/21, I started having headache followed by fever and chills in the afternoon and throughout the night till the early morning on 2/5/21 when the fever, chills and the headache are all gone. However, the site of the injection on my left arm has turned red. I would say it is a red circle with a radius of 1.5-inch around the injection location. It is painful when pressed on. Also my left armpit (lymph nod) is swollen and is painful. Hopefully I won't die :/

Shingles in L2 distribution on left side. Started with skin sensitivity and inguinal lymphadenopathy 1-2 days prior to emergence of characteristic lesions on left side of back in L2 distribution

Delayed reaction to the vaccine that began 6 days after first dose and persisted for approximately 3 days. I first became itchy on my abdomen and arms, I then developed flat oval hives and my eyes and upper lip began swelling. My throat did not swell. I took 2 Benadryl that evening that helped the itch and caused the facial swelling to subside somewhat. I slept thru the night and when the Benadryl wore off in the morning, the same symptoms returned. I took 1 Benadryl which again calmed the symptoms. The same symptoms returned in the evening and I went to an Urgent Care. I was prescribed Prednisone and more Benadryl. These medicines helped to calm the symptoms even more, but the itchiness and hives did come and go thru the next day. On day 3 I took Zyrtec in addition to my last dose of Predisone and the itchiness subsided completely. I took one more dose of Zyrtec the next day and the symptoms have not returned. I followed up with my PCM who tested for a Hazelnut allergy as I had eaten them that first night. She also referred me to an Allergist. I saw the Allergist who said in their opinion my immune system was having a reaction to the first dose of the COVID-19 vaccine and classified me as "medium risk" for the second dose. He recommended that I still get the second dose and to premeditate with Zyrtec the night before, day of and for 7 days following.

Pt received 2nd dose today at COVID shot line on. He was told to wait 15 minutes in lobby prior to leaving area. He states he began feeling a swollen lymph node on the right side of his posterior neck which then followed with swelling in both of his hands and pruitis on both arms.

Patient received vaccine on 2/25/21. Reported side effects started on 3/5/21- generalized hives or itching and swelling of lips, face and throat.

Swollen painful unilateral cervical (neck) lymph nodes (rock hard firm) on side of injection . Lasted for two weeks gradually dissipating in firmness and tenderness without any treatment

On Day 6 post vaccination, noticed slight tenderness near injection site. On Day 7 a rash developed and it was warm to the touch. On Day 8, some pain developed in addition. On Day 9, the rash grew in size and was itchy. On Day 10, it was almost cleared up. On Day 11, it appeared gone, On Day 12, the rash *re-appeared* but had grown in size. On Day 13, the rash was even bigger - had spread further down my arm. By Day 14, it had almost disppeared. On Day 5, I noticed a swollen lymph node near my left clavicle. On Day 6, this lymph node had swelled and now there was pain. I also experienced slight discomfort under my left armpit. The pain associated with the clavicle lymph node lasted until Day 8. As of Day 14, the lymph node is still swollen.

Inflammation started on the right and left sides of my neck two days after the shot. The pain was dull and lasted for about two days. The pain then moved to the middle of my back at the spine and blades of my lower back shoulders. The pain has lasted for a little over a week now and has lessened over time but is still noticeable. If I tried to sneeze or inhale deeply, this was difficult due to the discomfort I felt in my back and the sides of my neck.

Delayed onsert hypersensitivty to vaccine. Occured 3 days following second dose. Large swelling area on upper arm. Blistered rashes on ankles, tops of feet, both sides of hands, groin. All bilateral.

I had three symptoms: First, 4 days after the injection, the glands in my armpit were swollen and painful. It was a mild pain that lasted 3 days. Second, 7 days after vaccination, itchy and hot area of hives developed around the injection spot and slowly grew into a 6 inch circle. Third, at the same time with the second symptom, I am having a mild sciatic pain in my right leg. Symptom 1 has subsided, and symptoms 2 and 3 are still present, but seem to have slowed down.

2nd dose received 40 days after 1st dose. Within first 24 hours: Fever/Chills next morning. Stomach/body pain. Head pain. Pain radiating to testicles. Entire body aching. Couldn't find comfortable position to sit/stand/walk. Worse than any illness I've had, but without the brain fog Within next 48 hours: Armpit swollen on right arm (same as vaccine arm). Pain in head persisting. Pain behind ears. Dizzy/lightheaded feeling. Third Day after vaccine: Armpit is still swollen. Headache persists. Ache in muscles and joints seem to be re-emerging. But, overall, I'm fine and can work today. Note: Pollen count high - and I typically do get headaches from pollen - so, headaches could be attributable to pollen, but timing is suspicious.

Swelling or welt under both eyes. Blister-like bumps on swelling when I 1st noticed it, but they have lessened. But the welt is still there, and I feel the swelling more so now than I did last night. It's now been about 38 hours since receiving vaccine. It doesn't seem to be getting worse, aside from now being able to feel the swelling. Also, like I said, the blister-like bumps, on the welt/swelled area, have dissipated, so that got better. My face is also breaking out with acne since the shot. I do occasionally get acne break outs tho.

The patient received his 2nd dose of Moderna (LOT#, site, time unavailable) at an outside clinic the morning of 2/10/21 and presented to the ED with coughing and hypoxia (sp02 occasionally dropping into 70's) at 8 PM that day. He had quadriplegic spinal paralysis as a result of a remote MVA and has been hospitalized and critically ill in the past due to recurrent UTI's and pneumonia with associated sepsis. CT negative for pulmonary embolism and showed bilateral infiltrates. Clinical presentation consistent with bilateral pneumonia and started on Rocephin/azithromycin. He was initially stable in the hospital on 1-2 LPM 02. The following evening he became febrile and acutely developed asystole without any pre-existing arrhythmia. Resuscitation was attempted for 45 minutes but unsuccessful. Overall, I suspect his death was related to bacterial pneumonia and resulting acute respiratory failure, complicated by his quadriplegia and autonomic dysreflexia but reported this event as it did occur within 2 days of receiving his 2nd Moderna vaccination.

I began to have a fever with chills and very severe body aches that lasted 3 days after the shot and developed the day after the shot was administered. In addition, I developed a rash on my arm and was bedridden. I took a small walk that day and proceeded to swell up on the left side of my body but it subsided within a few hours. The next day, I suffered a stroke due to blot clotting and the clot flowing to my brain. I had to go to the hospital and was there for a few days.

The adverse reaction begin with dizziness, light headiness and increased body temp. Upon being released from vaccine location while driving an allergic reaction occurred causing swelling of the throat, skin change to red and rash like. I called 911 as I had trouble breathing and an ambulance took me to the hospital and administred epinephrine and benadryl. They called it analphlatic shock or extreme allergic reaction.

Received Moderna vaccine at 10:30 am on 4/21/21. within 15 minutes felt tightness in throat and "flush" from the neck up. One hour later became diaphoretic and arms swelled. The swelling to arms is similar to the reaction he has with pecan allergic reactions

Mildly swollen red splotches/bumps on several fingers (and underside of left big toe). Only mildly itchy unless scratched - then they become extremely itchy with burning sensation. Skin issues still present, but discomfort relieved by antihistamine (generic Zyrtec). Ibuprofen/lotion/etc had no effect. I believe it was a delayed vaccine reaction because I?ve never needed antihistamines before in my entire life - no known ?true allergies? (just significant GI issues with eurythromycin, so I avoid it).

Body aches and febrile illness same day as vaccine administration. Fever of hands and feet the following day that subsequently spread throughout bilateral lower extremities and trunk. Upon evaluation in hospital emergency department, found to meet criteria for sepsis without apparent source of infection. Tachycardia, hypotension, mildly elevated white count and absolute lymphopenia with bandemia. Clinical case highly suspicious for MIS-A.

Patient receive their vaccine on 4/21 at 4:00pm. The patient was instructed to stay for 30 minutes due to a history of a severe allergic reaction. Around 4:20 the patient started complaining of feeling ?funny and off.? He was moved from the chair to the floor and his feet were elevated. Help was requested from providers and MD and NP reported to help. Patient?s blood pressure and blood sugar were taken and he was given oxygen. Pt had facial swelling that seemingly got better and then worse. At that time epinephrine was administered and EMS was called. Pt was taken to ER for further evaluation.

Patient called pharmacy on 4/26/2021 stating that he had swelling on the face. I advised to seek medical attention and reporting to CDC using the V-Safe app.

Moderna COVID-19 Vaccine EUA Tenderness and swelling of the lymph node in the same arm of the injection site

Muscle soreness onset in 6 h, lasting for 2 days, self-limiting unilateral left axillary lymphadenopathy onset on day 4, lasting for 3 days, self-limiting.

Shortness of breath, swelling, diaphoresis. Diagnosed with myocarditis and acute heart failure which led to cardiogenic shock.

Had an emergency appendectomy on May 9th, 2021. Woke up on Friday the 7th with mild stomach pain from my chest to my belly button. It felt like indigestion. Pain stayed mild until Saturday evening when I started vomiting and had diarrhea and by 11:00 pm I went to the ER where they did bloodwork and a ct scan. Bloodwork showed high white blood cell count, scan showed appendicitis. After 4.5 hours there, I had to leave to go to another hospital and wait for a bed. After being in intense pain for a few more hours, I finally had the emergency surgery at 11:00 am Sunday the 9th.

Fevers, facial swelling Dx: Soft tissue swelling overlying the anterior maxilla, greater towards the left, consistent with cellulitis secondary to odontogenic infection. Abscess overlying the facial side of the left anterior maxilla at the level of the roots of the 1st and 2nd incisors measuring 0.7 x 0.4 x 1.3 cm. 1.4 x 0.8 cm periapical cyst of the left maxillary 1st and 2nd incisors.

Fatigue, minor swelling feet and hands occurred a week or two after second doses of vaccine. Hands and feet symptoms seem get a bit relieved in motion. Occasional heart pressure.

on 6/17 patient developed L shoulder pain with progressive cough, SOB, and inspiratory chest pain. ER admission discovered large pericardial effusion, low grade fever, elevated ERS, CRP, WBC.

On 6-24-21, the patient came in to inform us that he could not get his 2nd dose of moderna on the advice of his doctor. He said about 45 minutes after he received the vaccination, he started experiencing chest pains. He went to the emergency room. He said they did 2 E.K.G's, 2 chest X-rays, and blood work. He said they could not find the cause. He said the day after the vaccination he started experiencing shortness of breath and 1 week after the vaccination he started having swelling of the extremities. He said his doctor doesn't know if the vaccination is the cause but warned him not to get the 2nd dose. He says to this day he still experiences a dull ache in his chest, shortness of breath and swelling of the extremities. He said he is seeking treatment from a cardiologist. He said he is concerned about myocarditis.

34yo Male complains of a lymph node persistent for several months, which first presented several days after his 2nd Moderna COVID-19 dose. Physical examination remarkable for a pronounced soft, mobile, and non-tender 2x2cm supraclavicular lymph node above the midshaft left clavicle.

Delay Skim reaction. I initially only had normal fatigue and achy ness for 24 hrs starting day of vaccine. 9 days later my hands became itchy, irritable, swollen and darker shade of red. It was so irritable I couldn?t sleep and antihistamines, and otc itch creams didn?t provide relief, so I went to a clinic. Prescribed oral and topical steroids. Initially it allowed for itch relief so thought it was great. But next day it comes and go?s in all different spots of my body. It will start itchy then turn red, and could form bubbles on my skin. I apply topical steroid and it will go away about 30 minutes later and show up in another spot. Alternating from hands, formarms, armpits, lower stomach, upper thighs, but can come one and disappear on its own. I?m on my 5th and final day of oral prednisone 50mg a day, and still experiencing this. No other causes could have caused this. Nothing new during this time frame.

Lympadenopathy right above left clavicle. Symptoms started on 1/2/21 and have continued. Nothing has been used to treat the symptoms.

After 5 or so days I developed my first outbreak of dyshidrotic eczema. I?ve had the tiny bumps appear before but this time the itchy sensation was more intense and i itched which made a rash on my hands , feet and left elbow develop. I?ve had it for a number of days now.

1/4/2021: swollen/hard inguinal lymph nodes (right and left side - harder on left side). Pain scale: 2 out of 10. Remained tender and swollen for 2 days. 1/6/2021: Significantly swollen axillary lymph node in left armpit. Discomfort. Pain scale: 2.5 out of 10. Tender and uncomfortable with arm up and down.

Patient reports receiving the Moderna COVID-19 vaccine 12/28/20, started noticing itching to left proximal arm with subsequent rash developing to arm 48 hours ago. Patient reports lining rash last night, noted rash extending past lining today. Patient noted some reactive lymphadenopathy, denies any further concerns or complaints.

Patient reported noticing an erythemic area around injection site that began 7 days after his injection. The area was noted to be puritic and erythematous. To date, entire erythematous site is about 10cm by 4cm. The patient has left axillary lymphadenopathy but is otherwise asymptomatic. The patient encircled the area last night and noticed about 1 cm spread this AM. He took loratidine last night and this morning without effect.

Guillain Barre syndrome/AIDP event. Paresthesia and nerve pain developed in bilateral legs 4 hours after shot and progressed slowly for 4 days in intensity and area involved. Symptoms progressed distally to superior. On the 5th day symptoms progressed rapidly and involved bilateral legs up to the groin, left arm up to lateral shoulder, and right hand. I went to the hospital and was admitted to start IVIG treatment for Guillain Barre Syndrome/AIDP.

had myalgias and malaise on post-vaccination day 1. Had significant erythema and swelling at injection site and lymphadenopathy on post-vaccination day 6.

Became sick, dizzy throwing up, Kidney hurting. Went to doctor was sent to hospital was found to have enlarged prostate. Then had a lump come up under left arm same arm shot was given and was told my lymph nodes were swollen.

Covid arm reaction: exactly 7 days after administration of Dose 1. Swollen arm approximately 5 inches circumference, returning pain at injection site, intense itching requiring application of Hydrocortisone 1% CREAM. My arm started to hurt suddenly. It swelled up to the size of a tennis ball. It was not discolored. I iced t and it got hot to the touch. It showed a little bruising. The condition resolved within 4 days, The pain was not too bad.

Patient developed facial and oropharyngeal swelling approximately 1.5 hours after receiving the immunization.

Then this past Tuesday (03-23-2021) I went to receive the 2nd Covid-19 Moderna Vaccine at 1:00pm. I felt fine and had no adverse after this vaccination until 8:30pm the same day. At this time I started to have sever itching and within an hour the itching was over my entire body. It was a mixture of hives and regular pruritus/itching and areas of swelling throughout my body. This lasted all through the night into Wednesday, 03-24-21. Around the same time as the itching started Tuesday night, my Spinal joints, Hip joints, shoulder joints, knee joints, wrist and ankles became severely inflamed and very painful. On a scale of 1-10, 10 being the worst pain, at the highest I was at a 9. On Wednesday, I went in to my Primary Care for the itching and the pain. My PCP is on vacation this week, so I saw his Colleague. Dr. visibly recognized the redness from the itching all over my body, along with the extreme pain to my joints and gave me a thorough examination. After his examination he spoke to another physician who stepped in and also gave me an exam. Dr. advised the other Dr. and me of his initial hesitancy sending me home, wanting me to be observed overnight at the Hospital. After going to their Emergency Room. They gave me the choice but I truly thought it would go away over the course of the usual 24 hours post Vaccination; I also did not want to be exposed to any virus/sickness I might encounter/pick up in the Hospital due to my lowered immune system because of the rheumatoid arthritis injection Humira I take bi-monthly.. I explained that I was living with my mom, and our residence is 7 minutes from the Hospital, Emergency Room.) I assured both Physicians if it got worse, my mother would take me to the ER. So instead of going to Hospital, they prescribed a Hydrocortisone Cream of 2.5% for the itching, kept me on oral Benadryl and told me to stay hydrated. Also, Dr. said if it wasn?t better and/or worse Thursday(03/25/21)to come back in for re-evaluation. Along with the medications for itching, they prescribed a dose of pain medicine: Tylenol(APAP) #4 - or Acetaminophen with 60mg of Codeine. Yesterday - Thursday(03-25-2021) I was no better, and had to go back in to the Clinic. I was seen by Dr., who also believed this reaction to be based on the Humira along with my immune system naturally being lowered due to the AS and RA medication. Dr. examined me, as by yesterday my hands/fingers were hurting as well and itching was not any better. He prescribed Atarax (Hydroxyzine) 50mg Cap to be taken 3 times per day for the awful Pruritis, which has caused me to scratch so badly that I?ve had some bleeding from areas on my body, near the larger joints and spine. And also changed the Tylenol #4 to every 4 hours, basically more often for the pain associated. Adding the Hydroxyzine for the itch along with the Hydrocortisone, pain medication and time since Tuesday?s final vaccine, has helped a bit. Today, Friday, I am feeling less itchy but still having quite a bit of joint pain, but less than Wednesday or Thursday of this week. Because of the itching and joint/spine pain I received very little sleep until last evening when I was able to rest and not itch or toss and turn from pain. Hopefully on the mend.

Burning sensation emanating from lymph nodes that gets worse when patient lies down. This has been occurring for more than a week.

Started getting very itchy when I was working out/exercising on my waist, hands, and arms...Noticed some redness on those areas, and bumps around my waist. It went away after an hour. The next day, I was moving around and this time my face, stomach, waist, hands feet, legs,, and back,,went red and was very itchy. Itchiness seemed to start in the palms of my hands first then, moved all over my body. Started seeing bumps raise up on the tops of my hands and stomach. After I stopped moving around it slowly subsided. This has been going on for 4 days. Any time I move around vigorously, seems to trigger the reaction. Latent itchiness all over most of my body after the swelling and redness/bumps go away. Taking Benedryl 2x a day per PCP recommendation. Following up within 4 days if it doesn't improve.

My wife called the ambulance for me as symptoms I was having mimiced a heart attack. I had sudden and intense tightness in the left chest (felt like I was shot) the developed into pain and then numbness/immobility of the left arm (vaccine was administered in the left shoulder area). Since this point I have had routine soreness/tightness in the chest and constant shortness of breath/wheezing.

Infected hair follicles, resulting in swelling, with clear and yellow fluid that would crystalize after maybe 60 minutes. Sights included scalp, neck, arms, and chest. Lasted about 72 hours before symptoms ended.

Received 2nd dose of vaccine, 5 days later his left leg started hurting and swelling. He had severe headache afterwards, dizzy and blurred vision. 04/24/2021 he complained of not feeling right, coughed and passed out. Had fever and chills when he came to. Wife called 911 and was talking and walking. Was awake and talking in ambulance, oxygen level was low. Once arrived at hospital, he became restless and passed away within 3 minutes. Autopsy was performed due to his age - 35 and no pre-existing illnesses - was determined he died of Pulmonary Thrumbo Embolism

Immediate localized swelling, patient report of feeling sweaty and hot, then noted left arm felt cold, and felt it in his throat, later noteds "tasted it in his throat". Denied tongue swelling. He was given an injection of 0.3mg of Epinephrine 2 minutes after symptoms started, and 50mg IM Benadryl 3 minutes after the injection. His BP remained 130s-140s/90s, HR 90-99, O2 96-98% on room air. Patient was Alert and Oriented x 4, no tongue changes on exam, RRR no m/r/g and CTAB on cardiac and resp. exam. Hands felt cool to the touch and pale (bilatarally). He reported his hands and feet felt cold approx 10 minutes after the injection. The paramedics and ambulance arrived 18 minutes after the injection after which he was taken to the hospital

Continuing peripheral neuropathy. On 4/21 I received my first dose of the Moderna vaccine in my right arm and within two minutes had a vasovagal reaction?tingling in my underarm, flush feeling face and stiffness and tingling in my upper lips. That mostly subsided within 30 minutes as the dull arm pain set in. Over the next 7 days my arm pain intensified some and moved up my shoulder and down my triceps and into my elbow. 4/28 I awoke with numbness and tingling in my pointer and middle finger of my shot (right) arm in addition to the arm pain and neck and shoulder muscle tightness. 5/8 Arm pain and finger numbness still present, I did some yard work that exacerbated the arm pain, and at this point it was the first day I recognized the pain as a specific intense nerve pain from my shoulder, down my triceps, across my forearm and into my pointer and middle fingers. From this point on the nerve pain and finger numbness/tingling persisted and included intermittent cold forearm/fingers. 5/11 Met with my primary physician who diagnosed me with cervical radiculopathy (C7). Prescribed physical therapy, prednisone and flexeril. 5/12 Had a massage w/aroma therapy, cryotherapy, and cupping. Provided some relaxation, but no change in nerve pain or symptoms. 5/14 First day of PT; neck and arms stretches, neck traction and nerve glides 5/17 Completed Medrol dose pack, steroids seemingly had no effect. Flexeril at night helped relax neck and shoulder muscles a bit. 5/20 (Today) Still have numbness and tingling in my pointer and middle fingers with specific nerve pain down my triceps, elbow, across forearm and into my two fingers that intensifies with activity and at night. Intermittent cold sensation in right hand, underarm inflammation and tenderness, and general weakness of right hand and arm.

Pt developed aches, pain and headache and felt feverish day after receiving 2nd dose of Moderna vaccine. Continued on Sunday and on Monday developed leg stiffness and swelling. Patient presented to the ER with extensive lower leg DVT.

Patient presented to the office on 5/18/2021. Reported dyspnea, fluid retention, loss of appetite, and a cough productive of clear phlegm. The symptoms began after receiving his 2nd COVID vaccine on May 5th. Patient was tachycardic, tachypneic, and had prominent S3 gallop,, CXR significant for cardiomegaly.. An echocardiogram dated 5/21/2021 demonstrates severe left ventricular dysfunction with and EF of 25% and moderately reduced right ventricular systolic dysfunction.

Patient has swelling of right foot in and surrounding big toe with some redness. It is very painful and limits ability to walk. Swelling started Monday. He went to Dr. on Tuesday 06/01/2021. He has had gout in past, but his uric acid levels were normal.

On 27-Apr-2021, the patient received first dose of mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) 1 dosage form. On 02-Jun-2021, received second dose of mRNA-1273 (Moderna COVID-19 Vaccine) (unknown route) dosage was changed to 1 dosage form. On 02-Jun-2021, the patient experienced ANAPHYLACTIC REACTION (Anaphylactic reaction to vaccine) (seriousness criterion medically significant). The patient was treated with ALBUTEROL [SALBUTAMOL] on 02-Jun-2021 for Anaphylaxis, at an unspecified dose and frequency. On 02-Jun-2021, ANAPHYLACTIC REACTION (Anaphylactic reaction to vaccine) had resolved. On 02 Jun 2021, after the vaccination patient began to experience shortness of breath at 12:30 pm and patient's airways started to constrict at around 9:00 pm. Therefore patient went to the hospital. Action taken with mRNA-1273 in response to the events was not applicable. Based on the current available information and temporal association between the use of the product and the start date of the event, a causal relationship cannot be excluded. Swelling of the lymph nodes in the left armpit. The lymph nodes are still swollen about 6 weeks later

Had DVT and PE after the first dose of the Moderna Covid vaccine administered at First Health. This resulted in a two day hospitalization and was previously reported to VAERS. Pulmonary and hematology specialists were consulted and recommended that the second dose of the Moderna vaccine be given because the patient was anticoagulated with Eliquis. After the second dose was given at First Health, the patient had obvious worsening of the swelling in his affected leg three days later. We are currently reporting this reaction to the second vaccination. As a result of the recurrent, obvious worsening of the swelling in his left leg (known site of the DVT after the first vaccination), the patient was sent to the Emergency Department. There, he had a second doppler study which showed no worsening of his DVT in comparison to the previous study. He was advised to follow up with hematology and vascular surgery as an outpatient. He was also told to wear compression stockings and to elevate his left leg.

Low grade fever 100 degrees, general ill feeling, swollen foot, missed work. symptoms started the next day following the vaccine and was still present at the time he called the pharmacy late Tuesday 06/29/21

The patient received the Covid vaccination at 10:30 am on 7/6/21. At around 2 or 3 pm he said his arm was really painful and it lasted through Thursday. On Friday his arm was just tender and over the weekend his arm was fine. When he woke up today (Wednesday) his arm was painful again and swollen. It has a bump on it about the size of a 4x4 post it. He doesn't think it is necessary to contact his doctor at this time. He will try Ibuprofen and an ice pack and monitor and let us know how he is doing in the next 2-3 days.

Was in good health until I had my vaccine. I developed unknown chest pains and went to the ER. I was showing no signs of being sick and when I was admitted to the ER I was having an irregular heart beat and my white count shot up to 17,000. I was diagnosed with pneumonia and sent home. Since then to date I have developed a twitch in my left leg and I have not been able to produce sweat since my injection.

Underarm lymph nodes swollen about the size of a baseball. Also had fever and muscle soreness. Tried using ice and ibuprofen. Neither were effective in treating swollen lymph nodes. Tylenol was effective in testing fever and muscle soreness. Will see physician later today.

A 36-year-old gentleman seen today in consultation for further evaluation of unusual reaction following 1st coronavirus vaccine. He received Moderna recombinant MRNA vaccine on 12/24. On about 01/02, he developed a maculopapular pruritic rash that he refers to as hives present over the dorsum of both hands and predominantly right side of the face. He had some facial swelling. No breathing difficulties or other symptoms. He does not recall any rash, aches, fever. A few days later, he went in to the Community Health Clinic and received some prednisone for this, a 5-day course, and apparently, symptoms resolved by about day 4. No further problems, but he is wondering if he should receive 2nd vaccine. Again, no immediate symptoms of any importance following 1st Moderna dose on 12/24. No clear history of allergic rhinitis, asthma, eczema, bee sting allergy or food hypersensitivity. No problems with previous vaccines. He is even wondering if the vaccine and rash reaction are related and this is difficult to prove with any certainty.

On day 2 arm had redness and swelling but this was gone by day 3. Things went back to normal and patient concluded VSafe 7 day reporting. On day 8 the site began getting extremely itching and has swollen for three days and now has redness and rash on site of injection and is extremely warm to touch and itchy. Patient hasn?t reported to doctor yet as unknown whether this side effect is expected or whether it warrants doctor visit but patient wants adverse event to be known. Patient doesn?t consider this adverse to be severe yet but somewhere between minor and moderate adverse reaction.

?Moderna COVID-19 Vaccine EUA? Redness at and around site, swelling of site, pain at site, fatigue in arm, itching and and around site, swollen regional lymph nodes(clavicle).

Starting on day 7 after vaccine developed ?Covid Arm? rash with swelling, warmth and intense itchiness. This reaction has now lasted 9 days with varying intensity. Most media reporting with quotes form medical experts say it should have resolved after 24 hours and to contact a doctor if it lasts longer than 24 hours.

Area around left deltoid became red, it was not itchy though. Also, lymph nodes in the left axilla became swollen. I noticed these symptoms about seven days after the vaccine was given to me.

Swollen face and puffy, red eyes. Could not breath out of nose and felt a lot of sinus pressure. Took Mucinex, benedryl and used Afrin around 12p. Nasal passages opened and could breath out of nose, but facial swelling continues and nasal passages starting to get blocked again (6p).

Patient developed sudden onset vertigo with recurrent vomiting. He was brought to the emergency department and was evaluated under the stroke protocol and underwent CT, CTA, and MRI of the brain which did not reveal an etiology of his symptoms. He had a neurology consultation and the most likely cause was thought to be vestibular neuritis on the left side. He was treated with Zofran, Reglan, Meclizine, IV fluids, and Valium with partial relief and was discharged home with outpatient follow-up. Symptoms are unresolved at the time of this report.

Joint pain, muscle aches, pain at injection site, swollen axillary lymph nodes on same side as injection starting approx 8 hours after injection; additional symptoms following within the next 8 hours include fever, chills, GI upset (abdominal pain, nausea). Fever and chills resolved approx 36 hours following injection. Joint pain, muscle aches and GI upset persisted for approx 72 hours after onset, resolved approx 96 hours after injection.

Patient started experiencing lymphodema under left arm. Arm is painful to touch. Slightly red. Encouraged warm compress, Tylenol alternating with ibuprofen.

Fever of 101 degrees for 2.5 days, extreme muscle and body aches for 2.5 days, Moderate to severe swelling of the lymphnodes and armpit on right side for 5 days. 400mg of Ibuprofen taken ~ every 6-8 hours then tapered off to once a day.

The occurrence of additional non-serious events is detailed below. The patient's past medical history included No adverse event (No medical history provided). On 01-Apr-2021, the patient received first dose of mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) .5 ml. On 04-Apr-2021, the patient experienced ANGIOEDEMA (Angioedema of the lips) (seriousness criterion medically significant), VACCINATION SITE PRURITUS (Itching of the left arm) and VACCINATION SITE RASH (rash of the left forearm). On 06-Apr-2021, ANGIOEDEMA (Angioedema of the lips), VACCINATION SITE PRURITUS (Itching of the left arm) and VACCINATION SITE RASH (rash of the left forearm) had resolved. The action taken with mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) was unknown. For mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular), the reporter did not provide any causality assessments. Light swelling and tenderness for a day or two after injection. 10 days later injection site swelled twice as much as it originally did and a few lymph nodes in my neck seemed to be tender. Both went away completely after 2 to 3 days. Ps. The first number of the vaccine ?lot? number might be a 6. The hand writing is bad.

Patient received the moderna covid vaccine on 4/6/21. Developed general malaise and fever 4/7/21 around 1pm. Then developed acute sharp, pinching retrosternal, non-radiating chest pain at 9pm on 4/7/21. Chest pain was intermittent and nocturnal. no relief with nitroglycerin. He was admitted to the hospital 4/9/21 and started on colchicine 0.6 mg PO BID and indomethacin 50 mg PO TID with improvement. Troponins trended down with significant symptomatic relief following anti-inflammatories. At follow up visit 3 days after discharge (4/15/21) , patient reported continued improvement, reporting sternal chest pain 1 out of 10 . Also reported dyspnea that has improved since discharge from hospital but still worse than baseline.

On 4/13/21 patient developed difficulty walking and fell on 4/15/21. Also on 4/13/21 swelling and edema on both legs noted. Pt was admitted to hospital on 4/15/21 and found to have extensive DBT from lower IVC, Bilateral ileofemoral and both lower extremities. D-dimer was markedly elevated. Pt currently on anticoagulant Apixaban.

Urticaria and red erythematous, hot and blotchy rash all over body. Hot , swollen tense forearms. Chest tightness. Seen 4/13/21. Treated with Prednisone. Given 20 mg bid (prescribed for 5 days). Pt only took 3 days as rash resolved. Also advised pt to increase Pepcid to 20 mg BID and continue Zyrtec 10 mg he had taken for a few days. Rash completely resolved by followup visit 4/21/21.

The first dose was administered on 04/06/2021 at 6:45pm. The next day I was lethargic, moderate pain at the injection site, swollen lymph nodes in the neck, and about a week late developed a slight pain behind the knee on the right leg. 2nd dose was administered on 05/04/2021 at 6:15pm. Developed pain 2-3 hours in the right arm (injection site). Today 05/05/2021, woke up around 8am with pain behind my knee in the right leg. I have felt lethargic all day so far.

2nd Moderna vaccine administered on 4/29. (First dose 4/1). Arrived to hospital via ambulance on 4/30 unresponsive and hypoxic. Temp 102.2 . Patient responsive only to painful stimuli. Patient was intubated and admitted to ICU. Noted to have WBC 12.0 and 'patchy left lower lobe opacities suspicious for pneumonia" on chest x ray. Lumbar puncture positive for human herpesvirus 6. ( Reportedly receives Ocrevus q 5 months for MS). He was extubated on 5/1. Reports feeling fine the day before receiving his second COVID vaccine. After second dose he reports that he felt very tired and has no recollection of being transported to hospital. Patient is currently on room air with mild SOB awaiting transport to SNF.

He had first dose of Mederna COVID vaccine and then 2 days later developed sob and LE pain/swelling. Now dx with extensive femoral vein to pop DVT to calf and bilateral PE with right heart strain

3 days after I received my 2nd dose of Moderna Vaccine at the same facility (Which I received on 4/16/2021), I've had a series of what could be panic attacks. 1 on 4/19/21, 1 on 4/30/21, then another on 5/8/21, which then continues to stay with me on a milder intensity, but is lasting still today. In my previous 4 years of working, I've only missed a less than a 1 handful of days of work due to health conditions, but suddenly after receiving my 2nd dose of Moderna vaccine, I've had a myriad of what I initially thought were heart attacks where I was unable to drive to work (had to pull over twice on the road due to serious risk of being in an accident / having unprovoked panic attacks the morning of 4/19/21 and 5/8/21). Had extreme SOB and could seriously considered calling 911 on all 3 of my first episodes. Went to Urgent Care on 5/10/21, and have had several phone visits with my primary care physician since then and was placed on medical leave until 5/22/21, although I still am experience some side effects even today.

After dose 1 my left lymph node under my armpit became very large and sore. After dose 2 a rash has formed in different spots all over my my body. They locations of the rash are relatively small but they resemble a heat rash or even some could be confused with poison ivy. Most of the rash is located on the inner part of both arms and the front part of both legs . It is very itchy In all spots

pt reported significant left axillary swelling that was initially evaluated on 2/22/21 and treated as adenopathy with Claritin or Zyrtec. I saw patient on 3/24/21 after 2nd dose (given to right arm on 3/18/21), and at that time he was complaining of right axillary swelling with some redness as well as persisting left axillary swelling. Patient had also had other symptoms (fevers, aches, chest discomfort/cough) in the few days after the vaccines that had resolved. I consulted with our ID specialist, who saw the pt on 4/16/21, and at that time only left axillary fullness was noted.

Client received the 2nd COVID vaccine (Moderna, Lot # 027C21A, EXP: 07/11/2021) at 18:45. Both EMT's on site reported to PHN on site of similar reaction to 1st dose of Moderna. At 18:46 Client complains of "left arm getting tight/warm and swollen", after 5 minutes in observation, client's vitals were taken at the time of reported symptoms, blood pressure: 170/80, heart rate: 68, oxygen saturation 97%. Client mentions history of elevated blood pressure, allergy to ibuprofen, and stated "he forgot to take his daily medication." Per client previous symptoms with 1st Covid vaccine resolved and he did not take any medications or seek medical attention. Client is alert and oriented to person, place, time and date. Client denies nausea, chest pain, abdominal pain, shortness of breath, or any noticeable rashes or itchiness. At 18:52 vitals were taken; blood pressure 176/92, heart rate: 67, oxygen saturation 98%, Client requested an ice pack, ice pack given by EMT. Client stated "arm feels better with a cold compress and discomfort is improving." At 19:02 vitals were taken; blood pressure 170/94, heart rate: 71, oxygen saturation 97%, client continues to deny any other new symptoms, and stated "arm is improving". PHN on site advised client to follow up with primary care provider, offered EMS due to high blood pressure, client refused. PHN on site gave ER precautions. Client agreed and verbalized understanding about information. At 19:15 vitals were taken; blood pressure 152/84, heart rate: 72, oxygen saturation 97%, per client no new symptoms. At 19:19 client left facility with a steady gait.

Patient developed muscle and joint pains and fatigue roughly 3 weeks after the first dose of the vaccine. Developed swelling and pain in the hands. He developed difficulty walking due to joint pains. Received 2nd dose of vaccine on 5/28/21. Came to the ER on 6/4/21 due to persistent hand swelling. Labs sent on that day showed proteinuria for which he was sent to rheumatology (serology done for SLE came back positive) and nephrology. Patient has been diagnosed with systemic lupus eryhtematosus with renal involvement.

Patient came to get his first dose of Moderna vaccine, and anaphylaxis happens after 3-4 minutes. Patient seems to gasp for breath. He did not respond to call/touch. Pharmacist ran out to give EpiPen, then laid patient down onto the floor with his legs up onto the chair while the tech called 911. EMS came and took it over. Patient however denied to go to hospital.

16 hours after injection: Facial swelling around right eye for 18 hours. Hives (raised bumps) along left side of body and upper back area. Hives lasted about 6 hours, or about 3 hours after taking diphenhydramine 50mg. 12 hours after injection for about 8 hours: insomnia, tingling in lower legs, mild pain around injection site, stomach pain/nausea, headache, cough, stuffy nose, confusion/anxiety.

Employee called Employee Health office on 1/5/21 with complaint of lymph node pain to Left Arm. Referred employee to on the Job (Occ Med).

Headache and dizziness began 1 hour after injection and lasted for 30 minutes. 8 days after injection, redness/swelling/itching/lumps developed at injection site. Axillary lymph node swelling in arm injection was given.

Redness, swelling, hot, itchiness still present after 8 days. Urgent Care provider started me on oral Keflex. Very swollen lymph nodes to left axilla and near left clavicle. Moderate to severe shoulder pain. Unable to sleep on my left side. Inflammation to left shoulder.

Took shot on December 30th, 2020. January 3rd night I went to bed with bumps on the back of my head and itching pretty bad. I woke up sometime during the night and realized my top lip was swollen. When I got up the next morning my head was still itching and my top lip was still swollen. Monday, January 4th, 2021, I began having? Dime sized? Rep spots come up on my body all over that itched pretty bad. January 7th, my injection site turned really red and began to swell. I called my primary care physician and he immediately put me on some meds, both prescription and otc. The symptoms subsided January 9th. January 10th, I began to notice red spots began to appear again and itch. I continued the meds prescribed and thought it would go away, it did not and a few more appeared. Thursday, January 14, my left hand between my thumb and pointer finger began to hurt and turn red. A few hours later the same thing began happening to my right hand. They both began to swell, itch, and turn hot. This lasted all Thursday night and part of Friday morning, and then the swelling, and itching, began moving into both wrist and forearms. Both wrist and forearms turned red and were swollen mid way up the arm. The swelling subsided Saturday night, January 16th. The morning of Sunday, January 17th, my right foot became very tender and painful to walk. I removed my sock and had a considerable red rash on the inside and bottom of my arch that was hot to the touch and itching. A few hours later, a rash developed on the left side of my left foot, but not as painful or large. During this whole time, I have continued to have rashes and bumps appear all over my body, with them coming and going, usually lasting about 2 days.

Had pain at the injection site for 2.5 - 3 days only. After 3 days I observed both my lymph nodes in my armpits were swollen and I felt very fatigued. No real pain on lymph nodes just swollen. Lasted for approximately 5 days and returned to normal around January 20th. I am an athletic 37 year old male. Had no illness prior. Had telemedicine appointment with my doctor and he said to report the possible side effects. Also had blood work up done at his requests to confirm everything was ?normal? with me.

I was sore after the shot. I waited to exercise for a few days. On Monday I felt a burning sensation in my shoulder muscles. I discovered a lump under my left arm in my armpit area. It is painful. I also had euphoria upon receiving the vaccine.

Noticed minor pain and significant swelling of the lymph node underneath my left armpit. I am reporting it immediately. I have not sought medical attention, and likely will not unless the swelling does not decrease within a few days.

Patient noticed swelling in armpit region of the arm that received the vaccine. He was already utilizing ibuprofen and /or acetaminophen. Patient reports swelling seems to be resolving as of Wed 2/17/21.

Dose 1: aches; fatigue; chills within 24 hours. Swollen arm and tenderness 8 days after first dose. Dose 2: fever; fatigue; aches within 24 hours and lasting for 24 hours.

The patient had fever lasting 2 days and left arm pain lasting 4-5 days after the first dose. After the second dose, he also had a fever the first day, developed severe pain in the right arm with swelling and sensation of heat which did not respond to ibuprofen; radiation of pain down to his hand and into his upper chest, all of which persisted for several days, requiring a visit to the emergency department on 2/12/21, where a workup including CBC, chem, CK as well as an x-ray of his shoulder and venous doppler study were negative. His pain and swelling have persisted for over a month although they are starting to gradually improve.

Approximately 9 days after initial shot, I developed swelling (about 1 cm thick) between my elbow and shoulder near the injection site. The site itself was extremely red/rashy. The area was around 5-6 inches by 4-5 inches. It was itchy and hot to the touch compared to surrounding tissue. It was accompanied by slight pain in the arm. Symptoms receded by the 11th day.

Monday (3 days after injection) I noticed a lump on the lower left side of my neck; near my collarbone. Tuesday my primary care physician examined me and said it was likely a swollen lymph node, a possible reaction for the shot. It is tender to the touch but does not affect my day to day activity.

pt stated that after getting the vax his are was sore. He woke up next day and had developed a severe itchy red rash all of his entire body. His face and eyes are swollen. He took some Benadryl and went to work. His boss sent him home from work and told him to go to a Doctor. Pt called us to make a report and is on his way to Urgent Care in . He is continuing to get more reds dots on his body as we speak that are starting to blister.

Swollen lymph nodes very close to my right nipple, just above the nipple, with redish color and where I felt itch. That lasted around 2 days with some pain, then disappeared.

Delayed subcutaneous hypersensitivity. Swelling and itching eight days after injection. Original pain at injection site lasted four+ days, went away, then this happened overnight.

Adverse events occurred 10 -12 hours after the shot. - 101-degree fever - Chills - Body Aches - Swollen Lymph node in the left armpit - Headache - Nausea Symptoms lasted about 15 hours, the fever broke and the rest of the symptoms began to diminish. I took Tylenol to help with the symptoms. The lymph node in my left armpit is still swollen and certain areas of my body ache almost 48 hours later.

Very sore arm, fatigue within hours of the vaccine. Then 6th day (morning) throat swelling and face when waking up and rushing to the emergency room where I was given a steroid and Pepcid to open my airways and was discharged by 9:30 AM ET on 03/10/2021 after throat opened back up and swelling decreased. I was given a script for EPI, Medrol Pack and Pepcid. Upon returning home I had began having hiccups at 8 PM that lasted through Saturday 03/12/2021, where I was admitted to the hospital for a lot of antihistamine rash reactions over my body that continued from the morning of 03/10/2021. Then went to the hospital again on 03/13/2021 and was given a muscle relaxer baclofen around 7 PM to rid the constant hiccups that were concurrent from what seemed to be the steroid used to open my airways. Went home and have had fatigue, but feel mostly better with appetite back. My family doctor saw me on 03/16/2021 and saw that I was still having a reaction on my back and arms with antihistamines and said it was the Moderna shot and I needed to consult with a pharmacist on what to do or the FDA/CDC as he didn't know enough on it. Now Pharmacy won't give me my second dose due to the CDC guidelines and they want me in a medical setting to have the second one, if I am allowed - something I am not privy to. I need help and guidance ASAP on what to do.

Large Swollen golf ball size diameter swelling and discomfort that was hot to the touch. Treated with warm compress at first and then Iced for 20 minutes every hour with 600 mg of Ibuprophin and some antihistamine.

Within five minutes of the vaccination, the patient expressed distress. Patient was slumped over in his seat, extremely labored breathing and his eyes were rolling to the back of his head. The Pharmacist spoke to the patient to see how he was feeling but he wasn't very responsive so we administered an Epipen and immediaetly called 911. The paramedics came at around 6:20pm and checked the patients vitals. EMT said the patients vitals were normal and have the patient the option to go to the ER. Patient was alert and able to drink water. Patient refused hospitalization. We advised the patient to go directly to the emergency room if he was developing any symptoms once home and to follow up with his primary care provider.

Day 1-2, dizzy, muscle weakness, shaky hands, heart rate up and down all day, palpitations, cold hands and feet, injection site pain, pain in both arms and hands Day 3-4, muscle weakness, shaky hands, glands in neck swollen, cold hands, extreme muscle fatigue, tingles in feet. Day 5-30, muscle fatigue, hard to drive or type without getting tired muscles, shaky hands, pins and needles randomly in body, ache in joints, tight muscles, weakness right side body.

6 days after vaccine I developed painful hives along my joints, many on my hands, swollen lymph nodes, swollen tongue, both affecting swallowing, severe burning sensation on my tongue, numerous sores in my mouth including on my gums, and cold sores on my lips. As a result I?m unable to speak or eat. Recommended treatment has been Tylenol, and antihistamine. I?ve been taking probably more than recommended of both, and as of today, all symptoms persist.

Vaccine given on 5/7/2021 in the back of my arm in the fat portion. A bump formed on the back of my arm which was 6.25 inches long and 3.5 inches wide. I had 103.5F fever, nausea, body ache, and site pain. On 5/10/2021, the adverse events started. The injection site started itching, my face started itching, tingling and swelling in my face and lips. 5/12/2021, the injection site still itches, face and lips are still swollen. Took Benadryl gel capsules. I noticed as the swelling in my arm goes down, the other symptoms are going away.

Pt. reported to the emergency room c/o DIARRHEA and SWELLING OF LEFT FACIAL CHEEK. Informed dr. that he had received the MODERNA vaccine recently (10 days prior). their records did not acknowledge that the doctor identified the reaction as "allergic", but confirmed diarrhea as a diagnosis. Dr. informed pt. to proceed with 2nd Moderna vaccine, but to inform pharmacy of the incident so that he may be observed closely for a reaction.

Six weeks after second injection developed central serous retinopathy. Obtained COVID-19 test ~48 hours after diagnosis, results negative. Unsure if its related to the vaccine.

Right calf pain with leg swelling and heaviness over a couple of days then right medial calf pain with what appeared to be a bruise the size of my fist and a small central painful lump. Urgent Care appt. US done and found a medial distal thigh venous clot. started Aspirin 325mg once daily. On day 4 of taking this medication, my right leg completely resolved of the swelling and heaviness and the "bruise" completely resolved without any discoloration. This makes me think the "bruise" was engorged capillary vessels that were not flowing due to the clot.

Two days after getting the first dose of the vaccine I began to get headaches and sharp pains in my head that would come and go. My lymph nodes behind the back of my head were also sore. I was also very lightheaded and dizzy anytime I would bend over or when I would exercise. Unfortunately it has been over two months now and I am still having these issues almost daily. The sharp pains come and go throughout the day and usual occur in the temple area and one side at a time, and sometimes in the back of the head. The soreness in the lymph nodes behind my head varies day-to-day, and I still have some light-headedness that also varies depending on the day. I had no prior history of any of these conditions.

Patient called stating lymph nodes were swollen under neck and under arms. Under eye was a little swollen as well.

Within 30 minutes of shot, feeling of general comfort and a little tired. Within 2 hours of shot swelling of left neck lymph node, which subsided in less than 4 hours. Within 2 hours general itchiness felt on the skin with no visible skin reaction, subsided within 24 hours. Within 2 hours feeling of drunkenness with no alcohol consumption. According to my wife, I was even acting drunk. General fatigue for up to 2-4 weeks; however, persistent fatigue measured over two shots is hard to determine.

Patient developed fevers and malaise within 10 hours of the vaccine and then subsequently developed progressive shortness of breath and presented to the ED locally. Was thought to have a PE and underwent CT images which was equivocal. Transferred to our hospital where an MRI cardiac stress indicated myocarditis. Started on aspirin and colchicine and will require cardiology outpatient follow up. Patient is feeling better

Moderna COVID-19 Vaccine EUA Lymphadenopathy, axillary node on injection side. Palpable and painful, approximately 3-5 cm in diameter.

12/30/20 pressure in neck (like someone cupping it), mild 12/31/20 myofacial fatigue, jaw fatigue/weakness making chewing difficult 1/2/21 numbness and tingling in hands, worse left. Global mild analgesia 1/3/21 paresthesias in hands and feet. Legs weak, left worse than right. Severe myofacial fatigue. Headache. Malaise. Severe neck pain 8/10, general body pain 5/10 1/4/21: lymphadenopathy left deep cervical chain. Significant, visible through skin, easily palpable. Other symptoms improved 50% 1/8/21 (today) lymphadenopathy ongoing and unchanged, still severe. Assessed by PCP, felt to be adverse event. Still some paresthesia in hands, malaise.

I received the Vaccine on Wednesday 1/6/2021 . Later that evening, my right deltoid was slightly swollen and painful. I would rate the pain 5/10. The next day Thursday 1/7/21, I noticed intermittent heart flutter/palpitations. I had my routine morning coffee and I thought the combination of the vaccine and the coffee might be the caused. The flutter/palpitations continue to the present. I don't have shortness of breath. The left deltoid pain and swelling subsided. The flutter is intermittent and last 10 to 15 seconds.

Swelling, rash, and pruritis of multiple fingers of bilateral hands. Blisters on hands an feet, persistent now for 5 days. Using oral and topical benadryl and using topical corticosteroid with mild temporarily relief but subsequent reoccurrence of symptoms

Anaphylaxis (urticaria, tongue swelling, subjective difficulty breathing) starting approx. 24hrs first moderna dose. No prior episodes of anaphylaxis/allergic rxn. Treated with Benadryl 100mg PO (prior to arrival, pt administered), famotidine 20mg IV, Epinepherine 0.3mg IM. Monitored in ED, complete resolution of symptoms, discharged home.

No symptoms at time of vaccination and I only noticed the slight inflammation / bump in my left arm when I was going to bed on 1/21/2021. No pain, a little swelling (feels harder than normal tissue), and its slightly red.

Pt was vaccinated at 10 am on 1/25/21 He stated that at 3:00 am on 1/26/21 he began having severe chills trouble moving and extreme thirst. He started to experience tachycardia and was diaphoretic. He stated that at 8 am 1/26 he had tingling in his lips and they began to swell. He stated taking 50 mg of diphenhydramine orally at this time. He had swelling and tingling of his face, eyes and tongue shortly after the oral diphenhydramine. He called an ambulance and was taken to the ER. At the ER he was given IV diphenhydramine and kept 2 hours for observation. He was also given oral steroids. He was sent home 1/26 with prescription oral steroids. He stated he has continues to experience tachycardia with average HR at 110. He also continues to experience night sweats but stated this symptom is improving.

Same day of vaccination later on at night I got a fever of 105F, felt very nauseas, every time I stood up I felt like I was going to pass out & my blood sugar was high. This event lasted about 48hrs but I'm also experiencing swelling & problems with unration. I'm still waiting to get in contact with my doctor. Have not fully recovered from advers event waiting to get lab work done.

Vaccine received at 1600 on 2/4. 14 hours later on 2/5 at 0600, vomiting x 4, chills, fatigue, malaise, nasal congestion. Wife stated I was acting like I did when I had the flu. Took several doses of acetaminophen and naproxen with some relief. 2/6 at 0500, vomiting x 4, cough, nasal congestion, worsening malaise and restlessness, shortness of breath, transient chest pain, mild lymphadenopathy. Acetaminophen and naproxen no longer providing relief. Reported to urgent care clinic due to multiple symptoms not listed in vaccine information, was given ondansetron and promethazine. Recovered at midnight, approx 56 hours later, waking up to sweat-soaked bedsheets.

12 hours after shot, red swollen eyes, fatigue, body ache, tenderness and swelling of left arm. Shortly after chills, fever, swelling of lymph nodes, migraine, rapid heart rate, dizzy, nausea. Lasted about 36 hours. migraine, fatigue, body aches, selling of lymph nodes still present after 48 hours. 72 hours later bruised lymph nodes through out the body (left leg, right upper arm, and right rib cage.) Also noticeable joint and muscle pain. prominently lower back, and right rib cage.

13 hours after injection the left side of face went numb as well as partial tongue and left inner check It lasted 3 hours 11 hours after injection chills, fever, nauseous , sweats, and bone aches . Lasted 30 hours 48 hours after injection baseball size lump under right arm pit- swollen lympnode still there 2 days since it appeared 48 hours after injection blood circulation in hands and feet are not what they usually are they are blotchy and red and extreme sensitivity to cold -still ongoing

Standard side effects ( headache, fever, chills). Lymphnodes started to enlarge, and side effects continued to worsen. Eventually a blood test confirmed I had Mono.

Within 24-36 hours after I received my first dose of the Moderna vaccine, I developed headaches that then turned into swelling of the two occipital lymph nodes where my skull meets my neck. The lymph nodes swelled to roughly marble to golf ball sized protrusions in the occipital region of my head. This swelling as also accompanied by more swelling on the top right side of my head and also my skin began to break due to the swelling. Extreme headaches and swelling only started to subside on the right side of the back of my head after I received a steroid shot at an urgent care clinic. The steroid did not diminish the swelling and extreme pain on the right side of my head. A few days after this, I went to another urgent care clinic which diagnosed me some kind of infection due to the swelling and then prescribed me topical and oral antibiotics. As I right this, over three weeks from my first dose, I have scabbing and still some pain in the right side of my head.

Patient developed constellation of fever, chills, profound fatigue, and neck swelling 24 hours after dose 1 which improved with Tylenol and advil but persisted for nearly 4 weeks resulting in 6 day delay in his 2nd vaccination. During this time, he saw ENT for a chronic cough pre-dating his first shot and underwent CT to rule out sinus disease which revealed bilateral cervical lymphadenopathy. He then underwent 2nd dose and had exact same constellation of symptoms along with worsening of cervical lymphadenopathy and has persistent lymphadenopathy on exam at the time of consultation with infectious disease 3/16/2021) albeit symptoms and lymph nodes are improving.

Day #4 after covid vaccine #2 dose: mild sorethroat and mild neck enlarge lymph node - resolved in one day. Day #5: acid reflux symptoms, abdominal bloating - took Tums.. Day #6: chest discomfort with sharp pressure - worse with deep inspiration and temporarily relieved with belching/burping. Day #7: continues to have chest discomfort - worsening fatigue and some light headedness when bends over or standing up. EKG c/w acute pericarditis. Started on Colchicine. Day #8: feels much better and discharged from hospital on Colchicine 0.6mg twice a day.

On March 24, 2021, the day of the shot, I only felt soreness in my left arm. The next morning, March 25, 2021, I began to feel body aches, chills, and I was in a cold sweat. I took ibuprofen throughout the day to combat the symptoms. The evening of March 25, 2021, the pain was much more severe (body aches all over my body, including my face, teeth, my hands were sore and stiff, etc.). I did not sleep much of the night because the symptoms were very uncomfortable due to the chills and cold sweats. The morning of March 26, 2021, my left armpit began to swell. By the afternoon, my left armpit (the arm where I received the vaccine) was swollen, tender, and soft. There is some redness and the swelling and tenderness are more pervasive. It is difficult to hang my arm at my side due to the pain and swelling. I understand, after calling the nurse hotline, that this is a common side effect but I also have a skin condition (Hidradenitis Supparativa) and it is difficult to know whether flare-ups are triggered by environmental factors.

Felt like swelling under the skin starting from the injection site in the left shoulder. Traveled to my left hand fingertips. finally felt swelling in the left side of my face. Subsided within about an hour.

Patient's left ankle starting swelling; Patient's left leg starting swelling; A spontaneous report was received from a physician concerning a 38 years old male patient who experienced patient's left ankle starting swelling (joint swelling) and patient's left leg starting swelling (peripheral oedema). The patient's medical history was not reported by the reporter. Concomitant medications were not provided. On 11 Jan 2021, seven days prior to the onset of the event, the patient received first of their two planned doses of mRNA-1273 in the left arm for prophylaxis of Covid-19 infection. On 18 Jan 2021, the patient's left ankle and leg started swelling. Healthcare professional performed an X ray and other analysis on the patient but didn't find anything. The patient was treated with prednisone and the symptoms went away but when they stopped treating with prednisone the symptoms came back. Patient's symptoms were ongoing. Healthcare professional showed his concern as he doesn't know what was happening to the patient. The patient was advised by his physician not to received the second dose. Treatment for the events inluded predisone and Ibuprofen. The second dose of mRNA-1273 was discontinued in response to the events. Swollen neck, sore throat, ear pain beginning 2 days after vaccine. Symptoms still present after 2 weeks. Received antibiotics and steroids. Over 1 month after vaccination still have inflamed left sinus and feels like clogging in left ear. Went to ENT doctor and currently on stronger antibiotics and steroid.

In addition to the (common) side effect of chills and body aches, at around 75 hours after getting the shot I developed extreme gas pains and nausea while running, and barely made it home, collapsing on our floor. I then suffered severe diarrhea (which had not been listed as a possible side-effect). It is now five days out, and the diarrhea appears to have abated. But I continue to have lower abdominal bloating and discomfort. (I also have painful swollen lymph nodes under the armpit of injection, as I did after the first dose, for at least a week.) Note: These gastrointestinal symptoms are NOT like those I suffer from with gastric dysmotility, which affects my upper GI tract.

Swelling 1-3 days, fatigued all week with interspersed bouts of lots of energy (that could just be me); sore all over ( L arm / L neck / back - but again that could just be me and caused by carrying around a toddler); the injection site was sore last night, a little itchy this morning (3/5) and now I have a red bumpy rash (possibly in part from scratching). Also, bouts of diarrhea all week (not too unusually, hence possible dairy allergy referenced above).

4/17 - Substantial swelling of the lymph node beneath the right arm / behind the right pectoral. Approximately half a tennis ball protrusion. Accompanied by discomfort, but only dull pain. No visible redness or irritation at the lymph node. Redness and heat on the right upper arm injection site. No fever, no headache, and no swelling other than described above. Discomfort requires elevating the arm away from resting position. 4/18 - Swelling and fluid retention progressed down the right side long the rib cage. No substantial subsidence of the lymph node area, only additional swelling in the surrounding tissue. Began taking 600mg ibuprofen every 6 hours. Same requirement to elevate from resting position. 4/19 - No reduction in the swelling on the right side beneath the arm. Same dull pain requiring elevated arm away from the resting position. Visiting physician for physical examination.

Patient with cardiogenic shock due to severe myo-pericarditis, severe inflammatory response with high fever up to 104. Cardiogenic shock requiring placement of Impella support.

On Friday, April 2nd, I received my first dose of the Moderna vaccine. Within minutes, my heart started racing and I felt faint. Thereafter, my hands and face got the sensation of tingling. It felt like ice was running through my veins. My heart stopped racing approximately fifteen or so minutes after it began. The tingling sensation in my hands and face ended approximately one hour later. I also noticed a strange taste in my mouth during this same timeframe. Subsequently, I had the sensation of being dizzy and lightheaded. Importantly, I have never reacted to a vaccine in this manner. On Saturday, April 3rd, I woke up with symptoms that were to be expected. My arm was very sore and I felt a little achy. However, I continued to feel dizzy and lightheaded. That evening and for the next week or so, my heart was beating rapidly, and my blood pressure was elevated. I also had the continuous feeling of my skin crawling. My hands and feet also seemed to be experiencing tremors and twitching. On Sunday, April 4th, I woke up and noticed that my right eye seemed a little blurry as I read an article on my phone. I assumed I must have slept on my eye wrong and did not pay it much attention. I do not recall my eye being blurry for the rest of the day. The following morning on Monday, April 5th, I woke up once again feeling dizzy and lightheaded. As I walked to the restroom, I noticed that my right eye was blurry again. This time it seemed to be more pronounced. Moreover, when I closed my right eye, I could see hundreds of dots in different colors. In addition to having blurry vision, my legs felt weak and nearly buckled as I walked. I felt like I was intoxicated. Upon returning to bed, my teeth began chattering and I felt like I was freezing. I called my doctor?s office and spoke with a PA who recommended that I go to the ER. While I checked in at the ER, I could not remember my ZIP code. I felt like my brain was not running on all cylinders. Upon explaining my symptomology to the ER doctor, she recommended that I have my blood drawn and that I get an MRI to rule out a stroke. All test results came back normal. On Wednesday, April 7th, I made an appointment with an allergist. They tested me to see if I was allergic to Polyethylene Glycol (PEG) and Polysorbate 80. For the skin scratch portion of the test, neither of the substances had much of a reaction on my skin. I thought the Polysorbate 80 scratch had a little bit of redness to it, but it was negligible. When the doctor injected Polysorbate 80 under my skin with a syringe, a welt formed. I am not sure why, but PEG was not injected in the same manner. As a result of the test, the allergist confirmed that I am allergic to Polysorbate 80. He mentioned that there may be instances of cross reactivity to PEG for individuals that are allergic to Polysorbate 80. He recommended that I not get the second Moderna dose unless it was administered in a controlled setting. On Sunday April 11th, I noticed what felt like a hard vein in my abdominal fat. It felt slightly tender to the touch. I was fearful that it could be a clot. On Tuesday, April 13th, I woke up and as a I stood in the shower, my foot was painful to stand on. On top of my right foot near my ankle, I could visibly see red inflammation in a vein. I massaged the vein and the redness and pain dissipated. Being fearful that I did have blood clots, I scheduled an appointment with a PA at my doctor?s office that same day. We discussed my ongoing dizziness, lightheadedness, and me being fearful of blood clots. She had an EKG performed at the office and ordered a barrage of blood tests. Everything continued to be normal. On Sunday, April 18th, I again went to the ER, because the hard vein in my stomach became larger and more tender to the touch. When I bent over, it felt like there was a lot of pressure emanating from the hard vein down to my groin. Once again, blood was drawn and everything was normal. I also had an ultrasound performed on the area that felt like a hard vein and the doctor concluded that it was most likely swollen lymph nodes. I was diagnosed with lymphadenopathy. On Monday, April 26th, I scheduled an appointment with my primary care doctor to discuss the ongoing dizziness, lightheadedness, and pain in my stomach and groin area. He told me that he had seen other patients that were experiencing similar symptoms and that my immune system was possibly overreacting to the vaccine. He told me the symptoms should go away in time. He also ordered a new blood test to check for clots. The D-Dimer test came back normal. On Tuesday, April 27th, I experienced a painful genital herpes outbreak. I normally get one every year. I had just recovered from an infection in December of 2020. This outbreak was more intense and for the first time I had nerve pain that wrapped around my hip and down my inner thigh. As of a week later, the infection is almost gone. Today is May 4th, and I am still experiencing dizziness and lightheadedness. Some days are better than others and every time I get optimistic that my dizziness and lightheadedness is finally gone, it reappears. Being an IT consultant business owner and an attorney, I need to think clearly for my profession. The side effects from the Moderna vaccine have made it very difficult to function. I would like to get fully vaccinated, but I am fearful of getting the second shot. I do not want the vaccine to cause permanent neurological damage or worse. Finally, I am sure your organization is aware, but there are many other people experiencing the same symptoms as me.

Patient received the shot on 5/7/21 and by 5/9/21 he had swelling in lymph lode, under arm on the side he had received the vaccine.

The patient had very swollen glands and neck pain a few days after the immunization. Cyclobenzaprine and Methylprednisolone were prescribed to help on 1/25/2021. He was then prescribed some lidocaine patches on 02/17/2021. Finally he was prescribed celecoxib for pain and inflammation on 02/25/2021. His symptoms have lasted for about 3 months and with the help of the celecoxib his pain is not there anymore.

High fever that Tylenol wouldn't help, chills, uncontrollable shivering, muscle cramps/spasms, extreme fatigue/lethargy, loss of appetite, swollen glands, earache and congestion in right ear, acute joint pain, arm pain at injection site. All symptoms went away after 5-6 days.

Patient called and mentioned that he has lingering pain/soreness on his right arm after more than a week of getting his 2nd dose of Covid vaccine. He had his shot on his left arm. He said that he had some swelling also on his arm after his 1st dose but waited until his 2nd dose to inform us.

Pt was seen in the emergency department for substernal chest pain four days after his vaccination. He was found to have an elevated troponin. Echocardiogram in the ED was unremarkable. The patient was diagnosed with myocarditis and admitted for observation. Patient is currently stable and admitted.

Strong fatigue for 2 days. Strong joint aches for 2 days. Strong body aches for 2 days. Slightly enlarged supraclavicular lymph node on left side above collarbone. Still present. About the size of a pea. Mobile and painless.

I started with an itchy rash on my legs behind my knees. AT first I thought it was just bug bites, then it popped up on both my elbows, my hands and my feet. On my hands it was really swollen. It began a few days after my vaccination, I don't know exactly when. I contacted the DR on 28th or 29th of January, I can't remember the exact dates. The DR gave me cortisone as well as clobetasol and betamethasone (creams).The rashes went away in about a week after having medications.

3 weeks after initial dose (4/23/2021), I started experiencing dizziness and a constant mild headache. With the dizziness, there was no visual perception of moving or spinning objects and no proprioception problems. The dizziness is more like a sense of being unsteady and an inability to maintain balance. The feeling has never gone away and persists in any position. On different occasions, many times per hour, there is a sweeping feeling that is never in a persistent direction. On the first day of symptoms, the headache was moderate with a feeling of facial puffiness with an elevated blood pressure near hypertensive crisis levels... which was on a Sunday. The following Wednesday morning, I visited the Emergency room at 0034 EST 5/19/2021 because the dizziness was not getting better. A review of systems was performed with no abnormal results. EKG was normal. Cranial CT scan showed no indications of stroke either although I have not looked at it myself yet. Blood sugar level was at 90. Chem12 test was normal. I also have intermittent fog and below normal focus capacity.

On day 1 after vaccine (6/5), patient was exhausted/fatigued, had muscle pain and weakness, and had migraine/fever. These severe symptoms only lasted for the day. Day 3 after vaccine (6/7), patient noticed they had a loss of taste and smell; everything smelled like rotten eggs or a gas leak, everything has an unpleasant taste except water. Patient noticed on day 4 (6/8) that he has a swollen, inflamed lymph node on the right inside of his mouth. Patient is still experiencing loss of taste and smell, and still experiencing swollen lymph node in mouth.

On the 2nd dose, I decided to contact the doctor in advance. We cranked up the heater in the house. After getting the 2nd dose, I got sick the same way as the first dose. I was very cold and very weak. Thursday and Friday I'm very sick. I felt fine Saturday and Sunday. On Monday, my right arm swelled up so much that it couldn't handle it's own weight. The pain got really bad and I had to go back to the dr. I get naproxen sodium to take with Tylenol. Thankfully that worked. My arm still didn't work until later that Tuesday evening. I would not have been able to make it to the Dr. if I had to drive myself. The doctor called it Moderna arm and said it was something he has seen a lot of. Here is when I find out that something is not right. My mind starts getting trouble. If we went through the same thing in the winter, my heater would not be able to keep the house warm enough. I hear about the booster and I just know I can't handle it. I am not sure what to do about that.

On April 1, 2021 the symptoms presented with right shoulder pain. Over the course of several days bilateral paralysis presented in the lower extremities. On April 6, paralysis occurred in both hands/forearms. Then on April 7, bilateral facial paralysis was noted and was instructed to go to the local ER by PCP. A MRI of the brain and cervical region W/WO Contrast was performed along with spinal tap. On April 8, after a repeat MRI of the brain and cervical region, a diagnoses of Guillan Barre Syndrome was provided. April 8- April 12, 2021 IVIG therapy was administered while staying in the hospital. After the infusions were completed, I was moved to inpatient rehabilitation center on April 13. During my stay in the rehab unit I completed extensive therapy with OT/PT/SP along with neurophysiologist. I was discharged from hospital Rehab unit on April 22, 2021 (am). After discharge I was sent home and advised to continue outpatient therapy. I currently have paralysis in both legs, lower arms/hands, and right side of my face. I have been seeing a neruoopthamologist for my right eye as it can not close and is at risk for going blind. Currently I have not been given any sort of outcomes or time line as to when my condition should improve.

On 30-Mar-2021 at 1:00 PM, the patient received first dose of mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) 1 dosage form. On 27-Apr-2021 at 3:30 PM, received second dose of mRNA-1273 (Moderna COVID-19 Vaccine) (unknown route) dosage was changed to 1 dosage form. The pain then disappeared around May 20th, but it came back and much more aggressive and painful. I could barely get out of bed; my fingers and ankles were swollen. 12 hours after vaccine: Fever 104°F Day 2: 103° fever, no energy, body aches, chills, headache, swollen lymph node, pain in left armpit, sweats, slept all weekend.

Moderna Covid - 19 Vaccine EUA I received the vaccine on Wednesday January 6th. EARLY Friday morning around 30 minutes after midnight - I noticed pain on my left shoulder / neck area (tenderness). I began to feel around and found a swollen lymph node at the base of my neck just inside my left collar bone. Seems to be the supraclavical lymphnodes. I also noticed tenderness under my left arm in the arm pit region. I received the injection on the left side.

Dry itchy skin. Swollen limbs. Skin peeling. Redness. On both arms. Behind kneecap. Between legs. On shoulders, neck, and chest region.

Patient reported lethargy, hives, flushing and pain at injection site starting in the evening of 1/15/2021. Symptoms continued 1/16 and 1/17/21. On 1/18/21 in the morning patient noted lymph node swelling in the groins and still continued with intermittent flushing. Dr. made aware on Monday morning. On 1/19/2021 patient reported no further lethargy, site pain, or flushing but lymph node swelling persisted in bilateral groins.

Besides the usual and expected muscle pain, injection site soreness, and headache, I experienced a mild allergic reaction. Within 5 to 10 minutes after getting the Moderna COVID-19 vaccine (Lot#, handwritten), I experienced a mild swelling of my face and throat. I noticed the throat swelling when I drank water and also noticed my face tightening up. I have a severe allergic reaction to peaches, so I'm hypersensitive to these types of reactions, and I was a little worried. My throat and face were swollen for about an hour, but very mild compared to my reaction to peaches, and it subsided after about 2 hours or so.

Pain went away several days after injection on the 20th, but pain has returned and with swelling of my upper arm.

Around 430pm on the day of the shot I start itching really bad all over my body. After the itching my face start swelling, eye lids start swelling, hives in my face arms back stomach and side. No breathing issues. I took kids liquid Benadryl 3 teaspoons until my wife came from the store. Then I took 2 benadryl followed by 2 more 3:30mins later.

I received the vaccine on Friday 2-5-2021 and today is 2-9-2021 and currently still have a swollen lump in my under pit in the arm where I received the shot. I would say its a Swollen Lymph node.

39 year old male no known past medical history received his second dose of the moderna vaccine on 2/5. That evening he developed headache, malaise, fatigue, fevers. The symptoms persisted on 2/6 and 2/7 with some improvement in his headache on 2/7. On the evening of 2/7 he developed high fevers to 103, rigors, "vivid dreams" woke up with night sweats. On 2/8 developed pleuritic chest pain and shortness of breath and went to the hospital.

Patient reported watching the Nurse swab his shoulder with alcohol ( here shoulder meets the arm). He noted that he feklt the injection going in and muscle felt tense. He waited 15 minutes prior to levaing the area. He reports leaving area on or about 1030hrs. He states he informed spouse his shoulder was little sore but he return to work around 1230hrs. Patient reports as time went on his left arm became more sore that he had started to use his right arm to move his left. Patient reports he left work around 1700hrs, went home then to bed. Discomfort was rated 3-4/10. On or about 2300-2330 pain increased to 7-8/10 and he could not get comfortable. After mid night he went to the Emergency Room. During the evaluation the pain increased to 8-9/10. He was diagnosed with post immunization arthropathy unspecified site and released to home with instructions to use ice. He then notes after returning home he still could not become comfortable (stood in hot shower, sat in hot tub of water).On or about 0330 he states pain increased to 9/10 and then he went to the Emergency Room. Patient was evaluated, MRI was conducted which reveal: Fluid in the subacromial-subdeltoid bursa suggesting a reactive bursitis. He was discharged to home with instructions to f/u with his Primary Care Physician. Patient was seen by his PCP on 20210210 on or about 1445 hrs for revealuaton and treatment. Evaluation revealed increased discomfort with moving of (L) arm. He was plaed in a shoulder sling, a shoulder immobilizer was subsequently ordered; he was placed on Lortab 10, given Diclofenac 75mg and placed on quarters.

Moderna COVID?19 Vaccine Swollen lymph node in the neck, right side, size of a pea, no pain or discomfort

19 Feb: Received vaccinate roughly 1330hrs. [1400hrs through 2100hrs] lightheaded, slight headache. [1500hrs onward] tightness in throat. 20 Feb: [All day] tightness in throat; fatigue; glands swollen in jaw/neck; joint pain in shoulders and ankles. 21 Feb: [All day] tightness in throat; fatigue; glands swollen in jaw/neck; joint pain in shoulders, lower spine, and ankles. 22 Feb: [as of 1100hrs] tightness in throat; fatigue; glandes swollen in jaw/neck; joint pain in shoulders and ankles. Takes benadryl to alleviate sysmptoms, declines advise for further care.

Nurse/doctor hit blood vessel during vaccine injection. I bled afterwards but did not feel it. Immediately that evening I began to feel symptoms of fatigue. This grew into an overall feeling of sickness and 3 days later, I had major brain fog. This then developed into high anxiety. However after about day 6 I felt normal again. Then 20 days later, I am feeling the same symptoms again. My armpit is swollen and having brain fog and anxiety.

Headache and muscle aches the following day. Swollen and tender lymph node in the armpit of the arm the vaccine was administered in on the second day following the vaccine.

@ 3-5 min... arm started feeling a little sore, like a dead arm. That was the extent of things for a few hours. @ 4 hours... randomly while doing absolutely nothing started getting short of breath + breathing through a tube feeling + drastically increased brain fog. @ 11 hours... splitting headache + arm at the injection site painful to the touch + combination of pain/discomfort kept me awake all night despite my sleep Rx for insomnia. @ 17-18 hours later... leg muscles were sore and achy and cramping if I stayed in any position for too long. Began to feel feverish yet had no temperature (yet). My saturation which frequently drops but always rebounds fairly quickly stayed at about 90% for over an hour. @ about 20 hours... I began to get swollen lymph nodes and my temperature started climbing into the high 90?s (my ?normal? is around 97° so this made me feel like I was on fire). My SpO2 dropped to 84%... the lowest I?d measured it, even when hospitalized for a week back in December. Everything also felt heavy?every part of my body, anything I?d try to pick up... as of gravity had suddenly increased. @ 22 hours... low grade fever of 100.2° while on Tylenol... so w/o it likely would have been higher. @ 24 hours later... (330pm next day) it all really hit me... my saturation dropped to 79%. With otc cans of oxygen it only went up to 84% and kept dropping back to 80%. I was unbearably hot and uncomfortable and sore everywhere, but especially my neck and shoulders. I had zero appetite. @ 26 hours... chills so bad I couldn?t get warm when wearing a fleece, under 3 blankets, and head covered. @ 29 hours... so nauseous I couldn?t eat + my temperature (still with Tylenol in my system) peaked at 101°. @ around hour 30... so unbearably hot had to shed all the layers I?d put on + over me. My temperature was just barley back under 100° at this point (Tylenol still in system). @ 34 hours... Overnight arm pain + chills retuned to the point woke up from sleep Rx. I had to take more Tylenol due the pain. Got fully dressed with hoodie + fleece on all under my comforter in bed. Despite deep chills my forehead felt like it was literally on fire, but I couldn?t find the thermometer so who knows what my temperature was. @ 42 hours... dizzy + disoriented + saturation 86% + injection site pain. @ 48 hours... appetite still suppressed + injection site pain (though less than at worst)

burning at injection site at time of injection. burning that night and sore to move it for about two days then it got better. On day 9 it was swollen and very painful to move arm. Ached most of the time and hurts to move arm. Burning stabbing pain. swelling of area of upper arm of about 3X6 area but no redness and no rash

On 3/22/21, I woke up at 6:30a to find the left side of my face and lips completely swollen. I also had a number of large hives acrss my right bicep and elbow, as well as a few smaller hives on my back and underneath my feet. This reaction was the same reaction I received when I went to the ER on 12/10/2018 for difficulty breathing and an elevated BP. The ER administered TORODOL and I reacted the same way, ending up in the ER the very next day due to the reaction. I believe, after much diligent research, the reaction I received to the COVID vaccine was due to the Tromethamine in it, as this ingredient is also in TORODOL.

The patient had a medical history of celiac disease and is a kidney patient (other kidney surgically removed). The reporter states that Patient's concomitant medications are gabapentin and duloxetine hydrochloride for chronic back pain. On 11 Mar 2021, prior to the onset of events, the patient received his first of two planned doses of mRNA-1273 (unknown lot number) via intamuscular route on left arm for COVID-19 infection prophylaxis. On 11 Mar 2021, anaphylaxis 3 minutes post receiving the first dose of vaccine the patient developed numbness and his eyes became swollen, then numbness of the tongue, lips and closing of the throat, which all happened at the vaccination site.The attending EMT provided 3 pills of oral 50mg Benadryl (diphenhydramine) and did not apply an EpiPen. Patient's blood pressure went extremely high (180/111mmHg) instead of dropping. At that moment, it was decided to contact 911. The paramedics placed the patient on a stretcher and loaded him onto the emergency vehicle to be rushed to the hospital. On the way, the patient was given an EpiPen. While in the hospital, the patient was given more diphenhydramine and injected with methylprednisolone. Patient was in observation for 4 hours and then released with a prescription of an EpiPen. It was reported that the patient was extremely sleepy due to the medications given. When the patient arrived home, he went straight to bed. The patient was hospitalized on 11 Mar 2021 for Anaphylactic reaction, Hypoaesthesia, Eye swelling, Hypoaesthesia oral, Hypoaesthesia oral, Oropharyngeal discomfort, Blood pressure increased, Pruritus and released on same day after 4 hours. The next day, Friday (12 Mar 2021) the patient got up around 10:00am. Then at 10:05am, the symptoms began to reoccur with numbness of the lips, tongue, closing of throat and extreme itchiness. As soon as patient's husband noticed, he did not apply EpiPen and drove the patient to the emergency room. The emergency room followed the same protocol with methyprednisolone steroid, EpiPen and diphenhydramine. On Saturday (13 Mar 2021) and Sunday (14 Mar 2021), the doctor was seen and the patient was advised to take diphenhydramine every 4 hours. Then this past Monday (15 Mar 2021), patient started developing shortness of breath and a resting heart rate of 135bmp. That day, the patient was taken to urgent care. There he was administered diphenhydramine and the patient's heart was checked. Urgent care agreed that his heart was acting up and his blood pressure was acting up for an unknown reason. Reporter states that the patient had been developing tachycardia for no reason for a week now. Since the tachycardia had occurred, reporter would check patient's oxygen (O2) levels. When the tachycardia had started, the patient's oxygen saturation (O2 sat ) would drop as well. O2 levels were reported to be 88% and resting heart rate of 103bmp. The reporter would let 5 minutes pass, recheck, and everything would return to normal. On Monday (22 Mar 2021), patient is scheduled to see a cardiologist who will place the patient on a machine to check his heart rate for 3 days to see what is going on. Follow-up with various health care providers was scheduled for the rest of the week. Nephrologist will be seen on Monday, Neurologist will be seen on Tuesday, and Kaiser will be visited on Wednesday. Reporter stated that even though asked to run bloodwork and images, the emergency room did not do it. Lab work was performed on Monday (15 Mar 021) which came back normal. A check for blockage of the heart, also had looked fine. The outcome of the event Anaphylactic reaction, Hypoaesthesia, Eye swelling, Hypoaesthesia oral, Hypoaesthesia oral, Oropharyngeal discomfort, Blood pressure increased, Somnolence, Pruritus, Hypoaesthesia oral, Oropharyngeal discomfort, Blood pressure increased, Somnolence, Pruritus, Tachycardia, Heart rate increased, Dyspnoea, and the patient's O2 sat would drop as well, O2 levels were reported to be 88% was considered to be unknown at the time of this report. The treatment medications diphenhydramine, EpiPen injection x 2, and Methylprednisolone steroid injection x 2 was reported to be taken by the patient. The events Anaphylactic reaction, Anaphylactic reaction/Anaphylaxis, Hypoaesthesia/Numbness, Eye swelling/Swollen eyes, Hypoaesthesia oral/ Numbness of tongue, Hypoaesthesia oral/ Numbness of lips, Oropharyngeal discomfort/ Closing of throat, Blood pressure increased/Extremely high blood pressure 180/111mmHg, Pruritus/ Extreme itchiness were considered meet seriousness criteria of hospitalization, life threatening, and medically important.. The events Somnolence/Sleepiness, Pruritus/ Extreme itchiness, Heart rate increased / 35bmp resting heart rate, Dyspnoea/Shortness of Breath, and Tachycardia /Tachycardia were considered medically important. The causality per reporter was not provided.; Reporter's Comments: This case concerns a 39 Y/O M hospitalized with a serious expected anaphylactic reaction and unexpected BP increased, oropharyngeal discomfort, eye swelling, hypoaesthesia, pruritus, and NS unexpected somnolence, tachycardia, heart rate increased, and dyspnea with O2 saturation decreased. Event onset 3 minutes after first dose mRNA-1273. Based on current available information and temporal association between use of the product and start date of the event, a causal relationship cannot be excluded

Rash on my right side, just above my hip. It's a large grouping of raised, red bumps about half an inch wide at widest point (the head) and trailing back towards my back, maybe 3 inches. Listed date of onset is unclear. May have begun Wednesday or Friday. It has not seemed to respond to Benadryl

Gingival swelling and erythema, rash to trunk and axilla bilaterally, extreme fatigue, shortness of breath, decreased appetite and diarrhea.

About 24 hours of fever, headache, chills, mild fatigue. Several days of right underarm pain and discomfort (assuming Lymph Node), which is just starting to fade away, but not entirely gone yet.

Extreme pain in heels of both feet within 24hr of vaccine injection. Difficulty walking and standing. Constant pain in both feet during daytime and nighttime. Particularly acute pain in morning after sleep. After 1-2 weeks, pain remained in heels of both feet, but also migrated into lower ankles. Swelling was present from the beginning of the episode, but increased over time. Pain and swelling abated 3 weeks after first vaccine dose for about a week. Second vaccine dose occurred 3-29-21 and pain/swelling returned. Pain in heels was still present at this time, but majority of pain migrated to ankles and achilles regions. Swelling increased by week 5 to become obvious Edema. Acupuncture did not help. Stretching did not help. Rest and elevating feet did not help. Magnesium supplement may have helped relieve pain, but I am unsure. Compression sock did not help. Ibuprofen did relieve pain but not swelling. By week 9, no relief from symptoms. Podiatrist x-rayed affected areas and found no structural issues, but was concerned with Edema. Administered steroid dosepack for 6 days. Symptoms immediately abated. No symptoms as of today.

History of schizoaffective disorder, otherwise well-controlled, developed worsening delusions and possible neuroepileptic malignant syndrome the day after his 1st vaccination and again after his 2nd vaccination. Had not had any manic or delusions in the 4 years prior. The day after his 1st vaccination he developed new delusions and hypomania that resolved after 2 weeks. After his second vaccination he was hospitalized the next day with mania, delusions, aggression, hypertension, tachycardia. Required chemical sedation for mania.

Fever, neck pain, swollen face. The second night after the shoot, I sweat all night long, I never sweat like that when the Air Conditioning is working, joint pain from my waist down.

Left swelling/inflammation and pain in the left supraclavicular lymph nodes starting approximately 6 days after administration.

Patient has no clotting history. He developed body aches, left arm pain, and fevers immediately after immunization, but improved. Two days later on 6/2, he developed left flank pain and fevers. On 6/4, he was evaluated in the ED, with CT scan showing partially occlusive thrombosis of left renal artery with areas of renal infarction. Cr remained stably at baseline. Initial workup was otherwise notable for WBC 17 and negative infectious workup. He was initiated on therapeutic Lovenox. On 6/5, CTA chest/abdomen/pelvis was performed to evaluate for other clots. Left renal artery was complete thromboses with evolving foci of renal infarction. No other clots were detected. He continued to have fevers. By 6/8, his leukocytosis, fever, and flank pain resolved. Renal ultrasound with doppler showed restoration of some flow in the left renal artery. He is to be discharged 6/9 on Lovenox 1mg/kg BID without patient Hematology follow-up. His coagulability workup was notable for positive LAC, FVII deficiency, and PET/CT without evidence of occult malignancy.

Within the first 25 minutes, my arm went numb and my hand started swelling. I had a rapid heartbeat. I was nauseous, my blood pressure was elevated. They gave me a check up. I had paralysis in my left hand and was not able to move it. I was taken to the ER and had to do motion tests. I am able to move it now, but I have difficulty. I still have pain in my hand and arm.

after a week or so i started feeling puffy eyes and face and was concerned about it. i went to the doctor and upon consulting he asked me to get the TSH test done. when the report came back the TSH levels were through the roof. my TSH score came back above 100 while i never had any problems with the Thyroid before. no family history either.
