Moderna

severe left sided testicular pain, lasting approx 3 hours. Pt unable to stand or walk for several minutes after waking up. After using the restroom later, pt felt some pain diminish, but is still tender. Scheduled visit with Dr to evaluate on 6/22/2021. Suspected Testicular Appendage Torsion. Today (6/23/21) small blue "bruise type" spot on upper left testicle.

Vaccine recipient had received a dose of Pfizer vaccine about 3 weeks ago at a clinic. Recipient present to this clinic with his mother on 6/25/21 for his second dose of Pfizer vaccine. The nurse discussed the Pfizer vaccine and accidentally grabbed a Moderna. She vaccinated the child with 0.5mL of Moderna vaccine IM. Immediately after the recipient was vaccinated, the nurse realized she had given the Moderna instead of the Pfizer. The mother was notified immediately, vitals were taken and the child observed for 30 minutes post vaccination. Child had no ill effects and was sent home with clinic phone number in case of any issues. A nurse from this facility contacted the parent on 6/26/21 to check on the child, mother reported that child had "a slight headache and his arm is a little sore". A nurse from this facility contacted the parent on 6/28/21 again to check on the child, mother reported no complaints of headache arm soreness, or any other complaints.

pt received 1st vax which was moderna and was supposed to be pfizer. on July 2, he recieved another shot of moderna instead of pfizer. he has severe headaches, nose bleeds, and weakness.

Moderna vaccine given out of the recommended age group. No adverse reaction noted per mother. Child had a sore arm.

Patient was given Moderna vaccine at 15 years old accidentally, which is less than the minimum age in the EUA. Patient was supposed to receive Pfizer vaccine, although he still would have been too young for that as well. Patient is a volunteer at hospital where this occurred. There were no adverse effects other them arm soreness

Patient was vaccinated before the 18th birthday with Moderna vaccine, a medication administration error parent reported that child had a sore arm following vaccination, but no other adverse effects

Student reported shaking and feeling like heart was racing. Blacked out while in car with parents. EMS was called to store 6/6/2021. Student had shaking all day on Sunday and diarrhea. Student continues to complain of stomachache's , lightheaded and chest pain to center of chest that comes and goes and is a stabbing pain
