Moderna

Patient received Moderna covid vaccine which is not yet approved for patients under 18. Pharmacist did not realize only the Pfizer vaccine is approved for under 18 at this time. Confusion arose from state's phased rollout of vaccine populations, where now all patients 16+ are able to be vaccinated but the limitations on which vaccine were not made abundantly clear. No adverse events/effects reported. Patient is in good health with no issues.

Scribe informed patient was "ready to receive vaccine." Patient was nervous and RN was consoling patient at time. She injected vaccine without verifying which type of vaccine was administered for the first dose.

has not had an adverse effect, but he was given the first dose under the age of 18. He was rejected on getting his second dose and we are up against the wall on timeline allowance to get a second vaccine. He is autistic. He has been acting out lately emotionally which was NOT happening in advance of the vaccine.

Was nervous about getting the shot. Felt clammy and hot. Felt weak and had a headache. A Code was called and he was transported to the emergency room for monitoring.

Saturday morning after vaccine was administered: Tennis ball size oblong lump noted between eyebrows at middle of forehead. Felt "funny", cheeks flushed, no apparent fever (not hot to touch). Lump itself was not red, painful or itchy. it was definitely "noticeable". Headache on the day he received vaccine, but no other reports of headache following. Tends to be anxious and has a history of "passing out" when leaving a dentist's office in the past.

Patient had memory losses and was seen first at our facility and then transferred to Emergency Room. Was diagnosed with "transient global amnesia"

At approximately 2 PM on Wednesday the 10th, I began to feel light headed, nauseous, and feverish. Color drained from my face and I also began to lose track of the conversation I was having. I couldn't keep up with the pace of the conversation and eventually realized I couldn't remember what we were talking about. Looking back on the event now, I could only remember what was happening for around. 2 minute span before I forgot it. After losing my ability to form new memories, I began to lose old ones. I would look through pictures and not recognize faces, listen to songs and not remember the artists, and lose entire gaps of my past. My parents described my memory loss as that of a concussion but, to be frank, I don't remember much of any of it. All of my past memories have returned, but I still have yet to regain my memory from 2 PM to 10 PM of that day.

MINUTES AFTER VACCINATION, PATIENT HAD AN INTENSE HEADACHE, AND FATIGUE. PULSE, BP NORMAL. MAINTANED CONSCIOUSNESS. GAVE WATER AND SNACK. WAITED IN STORE FOR ABOUT AN HOUR BEFORE LEAVING.

Patient passed out and fell over. He did not injure himself in the fall. Was out for 1-2 seconds. Was a little light headed and weak as he sat in the chair, but was recovered after 15 minutes. Reported being nervous about needles and failed to eat/drink much earlier in the day.

About 10 minutes after the vaccine, the patient was standing in front of the pharmacy with his mom and fainted. He landed on his buttocks and did not appear to hit his head. He immediately appeared conscious but disoriented. He had a steady pulse and no itching or rash or trouble breathing, but he seemed pale. EMT was called and upon evaluation determined that he did not need further treatment. The patient went home with his mother.

This patient had some dizziness after the shot and was apprehensive in receiving it. He claimed he felt dizzy and in seconds had slumped off of his chair. He was unresponsive for a few seconds only about 5-10 seconds and we gave him an IM Benadryl as a precaution. He awoke complaining of a funny feeling in the mouth and tongue but had no rash or redness or swelling to indicate an allergic reaction. We decide to monitor closely for an escalation of symptoms but to have a EpiPen handy, and call an EMT unit. His blood pressure was low at first at around 107/68 then quickly come up in about 10 minutes to 130/80. With no other issues we believe he had a vasovagal syncope but wanted the ER to make sure we were correct. I called and checked on him after he was at the ER. and the nurse stated that he was doing very well and that they had seen this happen with the vaccine at other site locations.

History of Present Illness: Patient is a 18 y.o. male with no pertinent past medical history who presents to the ED with complaints of chest pain. The patient notes that he started noticing chest pain this AM after he woke up that is associated with nausea. The pain is described to be in the center of his chest and is persistent. EMS was called who administered 2 SL NTG and 1 dose of ASA 325 mg. He notes that he received his 2nd Moderna vaccine on Thursday April 15th. He is currently a senior in High school. He had no adverse reactions to his 1st dose of the Moderna vaccine but after the 2nd dose he notes that he was having fever, malaise, body aches, and decreased appetite. He also tells me that he has been so fatigued that he has not really gotten out of bed in the last 48 hours. In regards to his oral intake he had (1) 16.9 oz bottle of water yesterday and nothing else but did have a glass of orange juice and some cereal this morning. During my evaluation with the patient he notes that he is still having chest pain that he reports to be retrosternal any 5/10 in severity. He denies any radiation into the left arm or jaw. He feels the pain is worse with movement and better when he is resting. He has had associated nausea and shortness of breath. He has been having low-grade fevers at home but denies any coughing, abdominal pain, diarrhea, dysuria, burning or itching with urination, congestion, rhinorrhea. He denies any tobacco use, drug use, alcohol use. In the emergency department initial workup is notable for potassium 3.6, magnesium 1.7, phosphorus less than 1, troponin 7206 with repeat at 10,212. CPK noted to be 380. Initial EKG was nonischemic. COVID swab was noted to be negative. After I evaluated the patient around 1520 an RRT was called due to hypotension and bradycardia. His blood pressure had dropped to 80/40 and heart rate was in the low 50s and he was symptomatic with diaphoresis and feeling as if he was going to pass out. 1L fluid was given running wide open. Repeat EKG did show some diffuse ST segment elevations concerning for pericarditis. Cardiology and ICU team were notified and evaluated the patient. Response is likely that the be vasovagal from acute pain. 18 YOWM PRESENTED TO PHARMACY FOR ADMINISTRATION OF 1ST DOSE OF MODERNA COVID VACCINATION. HE MENTIONED BEING "SCARED OF NEEDLES" BUT AWKNOWLEDGED THAT HE WAS ON BOARD AND WISHED TO RECEIVE THE VACCINATION. AFTER PT WAS IN OBSERVATION FOR HALF OF THE 15 MINUTES REQUIRED, HE STARTED LOOKING PALE AND CONFUSED. PT FAINTED, AMBULANCE WAS CALLED, PT AWOKE AFTER 5 SECONDS FROM WHEN HE FAINTED. NO SWELLING OF TONGUE, FACE OR THROAT WAS APPARENT. PT STARTED TO VOMIT. ADMINISTERED 25MG OF ORAL DIPHENDYRAMINE SYRUP. AMBULANCE ARRIVED, CHECKED VITALS WHERE ALL WITHIN NORMAL RANGE. PT WAS PICKED UP FROM FACILITY BY HIS MOTHER. CALLED PT TO CHECK ON A FEW HOURS LATER, HE STATED HE WAS DOING WELL.

PATIENT STARTED FEELING HOT, NAUSEA, TINGLING IN FINGERS AND FEET, HANDS WERE RIGID. HE WAS VERY UPSET AND CRYING. HE HAD PAIN IN HIS HANDS. EMT THOUGHT HE WAS HAVING A PANIC ATTACK

After 15 minutes checked on patient and mom (who got vaccine with him at same time and was sitting next to him) said he's experiencing some anxiety and felt funny. Asked him if he wanted to wait another 15 minutes he said no. Asked if they would like me to call 911 or emergency services they both said no. Both said they wanted to leave as sitting there was adding to his anxiety. I asked mom if she knew what to do if the situation escalated and she said yes and wanted to leave with her son and not wait an additional 15 minutes for observation.

Passed out; Hallucination; Chest tightness; Sore arm; Headache; This spontaneous case was reported by a consumer (subsequently medically confirmed) and describes the occurrence of LOSS OF CONSCIOUSNESS (Passed out) and HALLUCINATION (Hallucination) in an 18-year-old male patient who received mRNA-1273 (Moderna COVID-19 Vaccine) (batch nos. 039B21A and 008B21A) for COVID-19 vaccination. The occurrence of additional non-serious events is detailed below. No Medical History information was reported. On 30-Mar-2021, the patient received first dose of mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) 1 dosage form. On 29-Apr-2021, received second dose of mRNA-1273 (Moderna COVID-19 Vaccine) (unknown route) dosage was changed to 1 dosage form. On 29-Apr-2021, the patient experienced LOSS OF CONSCIOUSNESS (Passed out) (seriousness criterion hospitalization), HALLUCINATION (Hallucination) (seriousness criterion hospitalization), CHEST DISCOMFORT (Chest tightness), PAIN IN EXTREMITY (Sore arm) and HEADACHE (Headache). At the time of the report, LOSS OF CONSCIOUSNESS (Passed out), HALLUCINATION (Hallucination), CHEST DISCOMFORT (Chest tightness), PAIN IN EXTREMITY (Sore arm) and HEADACHE (Headache) outcome was unknown. Patient was hospitalized due to vehicular accident Concomitant product use was not provided by the reporter. No treatment information was provided. The patient received both scheduled doses of mRNA-1273 prior to the events, therefore action taken with the drug in response to the events was not applicable. Patient was involved in an MVA post vaccination. Patient stated that he felt "out of it" after leaving the facility and did not register that there was a vehicle in front of him that was stopping. Patient rear ended a vehicle at a stop light and reported that he was very light headed and "out of it". Pt had a fever of 102 degrees x48 hours post vaccination as well.

Fever, chills, body aches, dizziness, sore left arm, weak body, brain fog - these signs lasted from 9:30 PM, Tuesday 5/18/21 through 6:00 PM , Wednesday 5/19/21. On Monday, 5/24/2021, patient experienced 2 bloody noses. One when he woke up at 7:30 AM and then another at 5:15 PM. Bloody noses are not normal for him.

Experienced chest pain two days after shot. I did have covid-19 1 year prior to the shot but was sent to the hospital for myocarditis. I was in the hospital for 2 days, now in unable to exercise and cannot do any harsh work.

On 6/1/2021 patient presented to the Medical Center with C/O chest pain, diaphoresis, and nightmares during his sleep. Physical exam revealed mild tachypnea.

ABOUT 1-2 MINUTES AFTER THE SHOT, PATIENT WAS DISORIENTED, STRETCHED OUT /PASSED OUT FOR A MINUTE, HAD SHORTNESS OF BREATH AND WAS LATER TRANSPORTED TO THE ER

4/23/21 at around 1pm, felt pain in left shoulder blade and around left upper lung and ribs area. Felt like tissues around it was tearing and grinding. After an hour felt pain near heart area and experienced nausia and felt faint. Was taken to hospital around 3pm. Was screened and tested for Covid-19 - negative. XRay was taken - did not show any issues Was given strong anti-inflammatory via shot Was monitored for 2 hours. Sent home. Discomfort improved over the weekend and no longer experiencing any symptoms.

Patient received vaccine and 10 minutes into waiting period, he began to slouch in chair and he dropped his belongings. We got to him quickly and held him up while it appeared he was fainting. We lowered him to the ground and called 911 and his father that was in store . He complained of being very hot and dizzy. He began to recover fairly quickly, he drank some water and stayed on floor. His dad arrived and the 911 call was cancelled due to his improving awareness. He stayed in the lobby for 20-30 minutes. He felt much better and left with his dad in good physical condition.

The patient felt dizzy after getting vaccine (about 1 minute). Before getting vaccine he said he felt nervous with the syringe.He said he felt dizzy and his hands are numbing. We let him sit down and drink some water. He felt fine after 15 minutes and he left.

Endorsed acute chest pain around 1200 on 6/21 initially that seemed to get better. On the early morning of 6/22 at 0400, the pain woke him up from sleep with a 7-8/10 in severity. At this point it was extremely painful for the patient to breathe but he denies shortness of breath. Denies radiation of pain, felt better when he was lying down, worse when sitting up, and was extremely bothersome while ambulating. Presented to Hospital and was found to have elevated troponin ( 7.25--13.00--2.52). Transferred to another Hospital Cardiac MRI supported the diagnosis of myopericarditis. By the time of transfer to Hospital, hos chest pain had resolved. He was treated with a 6 week course of ibuprofen and colchicine.

Pt was administered 1 dose of adult epipen and 911 was called. Once EMT arrived they took him to the hospital to be evaluated. He appeared to recover by demonstrating the ability to stand up walk around and respond to questions asked by the EMT.

Immediately after receiving vaccine, customer appeared to slouch over and I held his head and talked to him as I was unsure if he was just joking in front of his family. He then sat up and indicated he did not remember slouching. I explained to him that his blood pressure probably just went too low and asked him to continue breathing and try to put his head between his legs. He seemed fairly alert but a few seconds later, experienced some body jerking and I asked his father to call EMS, which he did. Customer then seemed to be in a semi alert state and started sweating profusely. I continued talking to him and watched for a fall until the EMS arrived. EMS triaged him and took him from the store. EMS reported systolic blood pressure unmeasurable upon arrival and around 70 at time of taking him from location (approximate numbers)

Patient felt nauseous. He got up to go to restroom without telling staff. He apparently lost passed out on his feet. He fell but did not remember falling. He immediately regained consciousness. After feeling better, he sat in chair while he waited for family member to arrive. Upon leaving, he seemed to be better.

HEIGHTENED BLOOD PRESSURE AND HEART RATE, ALTERED MENTAL STATUS. EMS REQUESTED AND AIDED TRANSPORTED TO MEDICAL CENTER.

I was woken up at approximately 2AM January 28th due to the fact that I had sweat through my clothes sheets and was shaking. I had severe muscle weakness, a severe headache, and fatigue that has limited my ability to do daily functions. Since 2AM 1500mg of tylenol have been ingested, but my fever holds at 100-101 degrees. Time of writing this document is 11:20AM January 28th.

Patient started not feeling well the night after receiving vaccine. Next day patient had flu like symptoms, headache, anxiety, increased heart rate and trouble breathing. Patient also presented with rash located back of neck and back - red, painful, burning and itching rash - painful to sleep on back the night after receiving the vaccine

Patient fell out of chair in post vac after vaccination. Patient hit head on floor, loss of consciousness noted. No visible injury noted. Patient regained consciousness. Neuro functions intact. Pupils equal and reactive to light eyes open spontaneously. Motor response intact. BP 110/55, P 76, R 16, O2 99%. 911 and mother called. EMS assessed patient. BP 75/48, P 61, R 18, O2 99%. Alert and oriented x 3. EMS recommended transport to hospital. Patient refused to be transported to hospital, patient educated. Patient left AMA and released to his mother's care. Patient assisted to car in wheelchair. Stable condition, no visible injury, refused further care.

19 yo medically complex M with prior COVID infection in Dec 2020 who received first dose of the Moderna COVID vaccine on 3/4 as well as Epo shot (for chronic anemia secondary to CKD). Recently admitted for MRSA sacral abscess and had recently been discharged to complete treatment on oral Bactrim. On 3/6, developed worsening diarrhea, decreased urine output, facial and tongue swelling, and altered mental status. EMS was called and patient was brought to the ED. Upon presentation, noted to have hypotension and was fluid resuscitated. Initial blood gas with hypercarbia, and he was intubated for airway protection. CXR was non-focal. He was started on empiric vancomycin and ceftriaxone, although these were discontinued with negative blood and urine cultures. Repeat respiratory film array was negative. Patient intubated 3/6-3/10.

Patient got the vaccine at 1:06pm on the left deltoid, 1st dose Moderna. 2 to 3 minutes when sitted in the post vaccine area, started to felt lightheadedness, anxious, diaphoretic and feeling of passing out. At 1:14pm, VS#1 95%, 56 & 105/58. Snacks were provided and monitor for s/sx of distress. At 1:29pm, VS#2 99%, 76, 109/76 & 16. Per patient "he feels much more better now" Denies any lightheadedness. No s/sx of distress. Steady gait. Alert and oriented. Patient is with his dad to drive him home. Left vaccination clinic around 1:45pm.

Currently having it. I have a swollen face. Blurry vision. Difficulty standing. Shallow breaths. Headache. Chills. Body aches. 101 fever. Unable to sleep but very tired. Not able to breather through nose. Sore and numb right arm. Tingling of my limbs. Occasionally sharp pain in muscles. I got the shot at 11 and was just fine but I had work. I drove for a little over an hour and progressively my arm got weaker. I did physical labor until 10 pm and then started some school work, but found my arm almost unable to move and my mood was deteriorating. When I went to bed and lied down, it hit me. It is 4 am and it is still going strong.

COVID-19 Vaccine Moderna Lot #027B21A Patient received 1st dose of Moderna COVID-19 Vaccine @ 1324. At 1330 during the observation period patient was found to be unconscious in his chair. Patient was immediately aroused and assessed. Patient Vital signs were within normal limits and patient noted to be diaphoretic. Patient did not recall loss of consciousness and only recalled the period before the event and then immediately after when aroused. Patient had no signs of allergic reaction. patient did state he felt anxious before recieving his vaccine. Patient assessed to have had a vasovagal response and transferred by EMS to ED for further evaluation.

About 5 minutes after receiving the Moderna COVID vaccine, patient stood up to ask the pharmacy for a cup of water. His eyes rolled back and he gently fell to the ground. Opened his eyes almost immediately after, had a strong pulse and no shortness of breath. Patient states it was just nerves. 911 and store management was called just incase. The paramedics check patient out and released him. Patient's mom came to pick him up.

This patient received Covid19 vaccine. This PT has a history of Autism and was getting anxious post vaccination. PT's mother was accompanying him when he began feeling dizzy, sweaty, hard of hearing and tightness in his Left arm where the vaccination was administered. PT was immediately evaluated by EMS and Fire Rescue on scene. PTs condition began to improve. PT declined transport.

During post vaccination wait time. Patient noticeably anxious/restless/bouncing legs. Initial assessment patient reported feeling like he had a hot flash and arms and were itchy. Vitals taken every five minutes, potential seizure activity noted, patient positioned for safety on cot. EMS activated, hives noted on left upper shoulder and left side of face. Patient complained of shortness of breath and crying. Patient became pale and LOC and temporary apnea noted, 0.3mg EpiPen administered in left leg x 1 dose, by RN. After administration patient gasped became combative. EMS arrived and patient transported via EMS to ED.

pt had syncopal episodes after vaccination. pt is unsure of LOC or if he hit his head. no signs of hematoma or laceration noted to posterior head. pt vital signs stable HR 80 BP 122/70 7:35. vitals signs at 7:40- HR 81, BP 118/76. pt sts he is having difficulty thinking and answering questions, c/o headache.. report called to ED. pt stable on discharge, walked pt to brothers car. reported.

Within hours of getting the shot, patients left eye started turning red. He visited a medical facility and they diagnosed him pink eye and gave him antibiotic drops. The drops did not seem to be working and by that Monday he went to see the eye doctor. Eye doctor said it was viral pink eye and thought it was strange that this came about right after the vaccine and in the eye on the same side he was vaccinated. Over the next days and weeks, the viral infection moved to the other eye. Both eyes stayed swollen, red and tearing constantly. He has lost some of his vision and says that things far away are very fuzzy looking. The doctor said it was the worst case of viral pink eye he has ever seen and that he would have it for the long haul of 4 to 6 weeks. He had to do a medical withdrawal from college due to missing too much of the nursing program he is enrolled in. The pinkeye is still ongoing and we don't know when it will be over.

Patient had parked in the COVID vaccine waiting area, and was observed to fall forward onto his steering wheel within a few minutes of parking by 2 other participants waiting in the car next to him. One of them immediately notified one of the parking lot volunteers who came and got me within 1 minute of this observation. 2 students and I came quickly to the open drivers side door and began talking to assess the situation. Mr. Bannister appeared pale, mildly diaphoretic (sweating) and slightly confused as to the passage of time, but was able to state and spell his name and talk coherently about his circumstances. He stated that he was not allergic to anything that he knew about, and that he had no prior history of similar reactions to vaccines or medicines although he had not taken any shots in several years. He noted that he had not eaten anything since breakfast (now mid-afternoon). He described himself as initially feeling weak, faint, and nauseated with his vaccinated arm feeling heavy and numb. He also reported that he had a sensation of blurriness before passing out, but denied any spinning sensation, visual loss or ringing in the ears. There were no obviously visible abrasions or bleeding on his face or forehead. The nursing students brought a chair, and we elevated his legs and gave him some water to drink during the time we were monitoring his vital signs. He refused a cupcake because of the nausea, but as time progressed he gained facial color and was more comfortable in conversing. His hands had equal muscle grip and strength, visible fingernail beds were pink and his capillary refill was less than 3 seconds. Towards the end of our monitoring, he did accept some nuts and M & Ms to eat. A University Safety Medical Report was completed by the appropriate safety personnel. He was fully AAO x 3 at the end of our monitoring, and stated that if he felt better he would drive to his apartment (less than 5 minutes away) but would wait for his girlfriend to get out of class if he did not feel fully capable of driving. We encouraged him to eat something light when he got home, to use cool water/warm water soaks at his shot site, exercise his arm, and to take an NSAID for pain relief the next day. We recommended that he eat something light before his second shot (which had already been scheduled), that he bring someone with him the next time, and as a precaution to wait 30 minutes following the second shot. He left at 3: 15 pm driving his car without any apparent difficulties. An online report was completed for the incident 04/20/2021.

PATIENT RECEIVED FIRST FOSE COVID19 VACCINE AT 10:20AM TODAY, SECONDS TO A MINUTE AFTER THAT PATIENT FAINTED FOR A FEW SECONDS BUT RECOVERED IMEDIATELY NOT KNOWING WHAT HAPPENED. 911 WAS CALLED STAT AND ARRIVED WITHIN APROX 5 MIN. PATIENT WAS COMUNICATIVE IMEDIATELY AFTER INCIDENT, SAID HE FEELS GOOD NOW BUT DIZZY BECAUSE HE DID NOT EAT FOOD IN THE AM. FATHER WAS PRESENT. EMT NOTICED PATIENT'S BLOOD PREASURE VERY LOW AND TOOK PATIENT TO THE HOSPITAL RIGHT AWAY. PATIENT DID NOT WANT TO GO TO HOSPITAL IMEDIATELY BUT AGREED AFTER ADVICED TO DO SO. PATIENT DENIED TROUBLE BREATHING OR ANY OTHER SYMPTOMS WHILE PRESENT AT PHARMACY. PATIENT AND EMT LEFT SITE BY 10:39AM

Post vaccination, vitals at 1:50pm Pulse 81 BP 149 Resp 20. Chief complaint of headache and dizziness. Patient states he feels weird and would like to go to the hospital. Patient disposition by medical center ambulance with no other symptoms present.

Patient reported having having seizure 2 hours after having the moderna covid shot. He said he woke up and didn't know what was going on and went to the emergency room. That was all that was mentioned and it was several days later that he reported the incident.

I asked the patient to wait for 30 minutes in the waiting room after the vaccine due to his history of rash with the meningitis shot. 5 minutes after the vaccine the patient stood up and looked disoriented and confused. I asked him if he was ok and he said that we was dizzy and wanted water. I had him sit down. I had my coworker 911 and I got him a bottle of water. He was sweating but felt better after drinking water. He reported to the Firemen that he had not eaten or drank that day yet. I asked him if he was feeling anxious and he said yes. The paramedics arrived and they checked his blood pressure. They said he looked good and was ok to leave.

About 3 hours after receiving the vaccine I started to experience minor hallucinations (presumably due to a conflict with my Irlan Syndrome.

I have swelling in my under arm on my left side. its painful and makes it difficult to sleep or move around

FAINTING. PATIENT HADN'T EATEN ALL DAY AND WAS VERY ANXIOUS. SAT IN CHAIR, HAD A PROTEIN BAR AND WATER AND ISSUES RESOLVED.

After having the shot, hedeveloped left arm pain and did develop some lymphadenopathy in his left axillary region. He was dealing with nausea and dizziness, and the following day, he went to work, although he was feeling a bit ill. While at work, he was found to have a fever, so he was sent home. He has been having hot and cold flashes since then. He has had a poor appetite. He has had a dry mouth and trouble sleeping. One day prior to admission, he then developed a chest pain that became constant and persisted, so he came into the emergency department on 05/19/2021 for further care. He has developed myocarditis after receiving the first of his Moderna Coronavirus Disease 19 vaccines.

Patient became delirious, angry, and confused almost immediately after the second vaccine. He began throwing up at the sight or smell of food. Symptoms intensified throughout the day and he was taken to urgent care around 8:30-9pm on that same evening. EKG showed ?Pericarditis? and a CK level of 1,459. he was in ER overnight and admitted to the hospital on Friday, May 14th.

In April 2021, the patient received first dose of mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) 1 dosage form. In April 2021, the patient experienced ALLERGY TO VACCINE (severe allergic reaction) (seriousness criteria hospitalization and life threatening), SEIZURE (seizure like activity) (seriousness criteria hospitalization, medically significant and life threatening), TREMOR (significant amount of shaking and diaphoresis) (seriousness criterion hospitalization) and HYPERHIDROSIS (significant amount of shaking and diaphoresis) (seriousness criterion hospitalization). On an unknown date, the patient experienced ANXIETY (Anxiety is really bad) (seriousness criterion hospitalization) and INTENTIONAL PRODUCT USE ISSUE (afraid to receive the second shot). The patient was hospitalized on sometime in April 2021 due to ALLERGY TO VACCINE, HYPERHIDROSIS, SEIZURE and TREMOR. The patient was treated with EPINEPHRINE (EPIPEN) in April 2021 for Allergy to vaccine, at a dose of 1 dosage form, unknown. At the time of the report, ALLERGY TO VACCINE (severe allergic reaction), SEIZURE (seizure like activity), TREMOR (significant amount of shaking and diaphoresis), HYPERHIDROSIS (significant amount of shaking and diaphoresis) and ANXIETY (Anxiety is really bad) outcome was unknown and INTENTIONAL PRODUCT USE ISSUE (afraid to receive the second shot) had resolved. DIAGNOSTIC RESULTS (normal ranges are provided in parenthesis if available): In April 2021, Blood pressure measurement: went way up (High) went way up. In April 2021, Heart rate: went way up (High) went way up. The action taken with mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) was unknown. The patient received his first Moderna vaccine dose on maybe 02-Apr-2021 or 03-Apr-2021, possibly in his right arm. Within 5-10 minutes, he had a severe allergic reaction which consisted of seizure-like activities. The patient was treated with an EpiPen shot and sent to the emergency room. It was reported the patient had anxiety which was really bad and he was now being treated with an unspecified anti-anxiety medication to help. The patient was treated with DIPHENHYDRAMINE HYDROCHLORIDE (BENADRYL [DIPHENHYDRAMINE HYDROCHLORIDE]) for Allergic reaction, at a dose of 1 dosage form; EPINEPHRINE (EPIPEN) for Allergic reaction, at a dose of 1 dosage form and ASPIRIN [ACETYLSALICYLIC ACID] for Adverse event, at a dose of 1 dosage form. Reportedly, patient stayed at hospital for one hour until treatment made effect. The patient is thinking whether to take a second dose of moderna or not. Systemic: Fainting / Unresponsive-Mild, Additional Details: patient fainted immediately after receiving vaccine, seemed slightly disoriented initially upon waking up, felt nauseous, but symptoms rapidly improved after drinking water and eating crackers and remaining seated. Patient called mom to pick him up so that he was not at risk driving home alone.

Started feeling sick (light headed, anxious, felt like he was coming down with illness and chest pressure) on 518, by 6am on 6/19 he woke up crying with massive chest pressure, we took him to urgent care at 12:30p, immediately sent to ER due to abnormal EKG, admitted in ER with more tests & blood work taken(troponin was 1300) so he then was send via ambulance transport to cardiac unit at Hospital where he spent the night and on 5/20 they did a battery of tests to figure out what his diagnosis was. Confirmed a minor heart attack caused by a cardiac embolism. Echocardiogram showed minor heart damage. He was prescribed a ZIO patch to wear for 2 weeks resulting in no issues. Dr confirmed this event was Covid vaccine related.

The patient was fine immediately following the injection. I left him for about 5 minutes to get his immunization card. When I returned he was very pale and sweating profusely. He said he felt very dizzy and was quite frightened. I reached out to hold his arms and ease him to the floor when he fell backwards unconscious in the chair. I was able to keep him from falling to the ground and I called for help. He returned to consciousness after about 10 seconds and I applied a cold compress to his head and neck. He began feeling better rapidly and was able to drink water and rest comfortably. He was able to leave the area without further issue or need for medical attention

A few minutes after patient received the Moderna vaccine in his left shoulder, he reported that his left arm was numb down through his 'pinky' finger (although he was able to move it). He subsequently reported that he felt tired, he developed a headache, he had some stomach cramps and began to get nauseous. He did not ever vomit during this period. His blood pressure was monitored and found to be elevated initially at 173/121 (with a pulse of 69). His blood pressure was monitored over the course of just over an hour. It varied over this period but was elevated. The pharmacist in charge determined that patient was not able to go back to work. Patent stated that his girlfriend with whom he lives was off work today and could stay with him. He was advised to call 911 if he started feeling worse. Patient was driven to his home by a co-worker. Through the course of the day, contact was made with Patient and his girlfriend (per his authorization). He ate and then slept for some time. Upon awakening, he vomited for awhile - but "not alot". The numbness in his arm started subsiding. The headache maintained. They had no way to check his blood pressure. At 3:36pm he indicated he was "doing somewhat better" but the nausea was not any better.

Patient presented sweating, flushed, and shaking an hour after administration. Appeared to be in a panic attack

Feeling lightheaded and anxious; vitals obtained/ stable, po hydration, rest, monitor x 30 minutes symptoms improve/ feels better released from facility w/parent (mom)

After receiving vaccine, pt walked into the post vaccine observation room located outside the door of the vaccination room. Pt. sat down in chair, went to put his head down towards his knees, then patient slid out of chair and onto the floor. This RN was immediately at the patients side, and pt. immediately responded to his name. Pt. insisted on standing up against this RN's advice, and got back into the chair. Pt stated he didn't know what happened, so I explained to him that he had fainted. His mother and father were present. Pt stated he ate no breakfast today. Pt. states he has no health concerns, allergies, or issues. Pt.s stated that he is very afraid of needles and thinks he was just nervous. BP readings: 09:45 90/75 HR 60, 09:50 90/75 HR 60 10:00 BP 113/72 HR 65. Pt stated he was feeling fine. Pt. released to go home w/Parents at 10:30. This RN called this pt. to f/u at noon today, and patient stated he was feeling good and thinks he was just really nervous. Pt. thanked this RN for the phone call and call was ended.

Approximately 10 minutes after receiving the Moderna vaccine, this writer was called to the patient room by other staff. The patient was extremely confused and not speaking to anyone. He was gazing to the left and not making eye contact with this writer, who was loudly talking to the patient. Multiple attempts were made to get the patient's attention with no success. The patient then began to involuntarily raise his bent left arm in front of his face and then back down to his side. In this writer's opinion, the patient was having a silent seizure. This writer sat with the patient, who became aware approximately 15 seconds later. He was able to answer questions and was quickly reoriented. At this point this writer had the staff call 911 for further evaluation. After answering basic questions appropriately., this writer gathered vital signs on the patient . The first set of vitals were: bp 88/48, pulse 60, respirations 14/min, oxygen saturation 93% on room air, temp 98.5. After small conversation with the patient, another set of vitals were taken: bp 94/70, pulse 68, respirations 15/min, oxygen saturation 95% on room air, temp 98.8. We then had the patient drink some water, then had him lie down in the patient room until EMS arrived. By the time they arrived, the patient was completed reoriented and laughing with office staff. EMS evaluated the patient and deemed him safe to go home with his friend driving. This writer walked the patient to the car to assess balance and coordination. This writer informed the patient and his friend to call 911 if symptoms returned. The patient agreed.

Heart rate reg. No edema, pain or redness at vaccination site. enc to drink water. 1515 Assisted to sit in chair x 10 mins. 1526 Amb. in store. 1535 States he will leave to go home now. Home in stable condition.

Moderna COVID-19 Vaccine EUA. I started feeling body aches and fever chills around 11am the day after I received my first dose. I had to leave work early due to the symptoms.

After recieving his Moderna vaccine the pt devoloped pallor, complained of being light headed and that his thorat felt "weird". I checked the pt's pulse, it was normal. He complained he was feeling worse, I instructed him to stop drinking water and to lay down and elevate his legs. I had a staff call 911 as the pt was complining he was having trouble breathing. The paramedics showed up before I could check his blood pressure. They checked his vitals then motiored the pt for 15 minutes. They then cleared him to leave when he felt ok.

Lightheadedness- pale - appeared dazed- HR 59 B/P difficult to hear- pulse weak - monitored for approx. 30minutes BP high to 90/60 and HR 66- 02 Sats are 100% encouraged fluids and a meal- he was smiling, joking, feeling better when he left the site

I had pretty bad chills and a headache the night of that lasted the whole night and part of the next day. It made it very hard to sleep

patient was nervous after the injection-stepped out of the immunization room and sat in chair- felt light headed and passed out for about 5 seconds

Patient started to feel chest tightness and tingling in his left arm. It then started to travel down his left leg. His arm was cold and numb at one point. He was concious but very confused and could not think straight. He had someone drive him to the ER. They did a CT scan, EKG, and other routine lab tests and all were normal. They attributed it to the vaccine.

At 5:00pm. Blood pressure was elevated to 137/91, No temperature noted even though he felt hot and clammy. Gave him water, and put legs on a pillow and he lay down At 6:30 he began to feel sweaty, clammy and included nausea. These episodes continues until about 8:30pm when he started to feel a bit better After a night of sleep that was restless and about 6hr long woke to feeling better but tired.

Patient had vasovagal syncope ( due to anxiety) after being vaccinated roughly 5-10 minutes afterwards while sitting in post observation area. His mother was with him and propped him up from falling out of the chair until I and my other staff could assist. Propped his feet up with another chair, provided and ice pack and a bottle of water. Patient was fine after sitting for an additional 5-10 minutes.

Vasovagal syncope induced from anxiety. While waiting in post vaccine observation while sitting, patient fainted briefly while still in chair. Mother braced him until I and my staff could assist. Elevated legs with another chair and provided and ice pack and bottle of water. After 5-10 minutes, the patient was fine and left without any issues.

Felt dizziness and lightheaded about two minutes after receiving shot. I passed out and when I woke up I was struggling to regain consciousness control of my body as I was having a seizure.

Arm soreness, tinnitus is much louder, dizziness, nausea, insomnia, fatigue, loss of appetite, diarrhea, On 4/27/21 I went to urgent care, they ran blood tests which showed possible signs of viral infection, was only prescribed zofran for the nausea and vomiting. Ears showed no outward sign of irritation or infection

SEVERE LEG CRAMPING AND PAIN TO THE POINT WHERE THE PATIENT CAN'T SLEEP. PATIENT HAS MISSED WORK 4 DAYS SO FAR. CAN WALK BUT HURTS TO WALK.

After 10 minutes after administering the vaccine, the patient fell on the floor dazed. Face was completely red. After a few minutes he was able to respond. Called 911 and the ambulance arrived. The paramedics attended to him. He was able to stand and was assisted to the ambulance for further evaluation.

A few weeks after the vaccination, I was sitting in one of my college classes and out of nowhere my heart rate increased significantly and I felt extremely lightheaded/panicked. Within 5 minutes I felt normal. Throughout the next week I had several similar episodes and experienced chest pain frequently. I have never had these symptoms before.

patient lost consciousness about 30 seconds after receiving the vaccine. Initially I treated it as a syncopal episode and was just trying to wake him up but he began to shake (seizure-like) after about 15 seconds. I could not rule out anaphylaxis so I administered a dose of epinephrine via epipen. Patient regained consciousness approximately 5 to 10 seconds after epipen. EMS arrived approximately 10 minutes later and took patient to the hospital for further observation. Patient vomited on ambulance ride but did not lose consciousness again. Patient reportedly fine after being administered fluids but reports no memory of ambulance ride or even being vaccinated.

patient developed chills, headaches, fatigue and unable to do normal school work tasks for 2 days due to weakness and nausea.

patient had difficulty resting at night and had trouble breathing, was feeling somewhat better the next day though patient has a history of asthma. stated if felt worse later or symptoms did not go away would follow up with md

Awoke with incapacitating headache the day after the second dose. Headache has still persisted at the time of this report but has gone down in severity and is accompanied by a feeling of pressure in the head. Severe brain fog also started the day after second dose. Brain fog still continues but is intermittent and gets worse when other symptoms do. Profound fatigue started the day after the second dose and still continues and leaves me mostly housebound. Multiple episodes of post-exertional malaise have occurred in which symptoms are severely exacerbated after mental or physical exertion. I am now unable to exercise.

I felt extreme chills at night, and could not sleep even with a ton of clothing and layers on, and even after taking sleep medication. Also, nausea, and fatigue (but I think fatigue is due to lack of sleep)

Pt received vaccine, about 5 mins later he stated he was not feeling well, but could not articulate symptoms. He was breathing ok, but was in a state of panic. He requested we call 911, which we did immediately, and gave him an ice pack. EMTs arrived and he refused transport. He called his mother, who came and got him. EMTs waited with him until she arrived.

6/4/2021 1730 Pt called and stated that he felt fine the day of the 2nd Moderna vaccine, but when he woke up early the next morning, he experienced dizziness and his gait was unsteady. He went to work and began to feel worse. His co-workers noticed that he was not completing his paperwork correctly and he struggled doing his daily tasks. He stated he felt like he was going to pass out and was out of breath. His supervisor sent him home. He attempted to drive home and swerved off the road. He parked his car and got a ride home. He slept for several hours. He stated he is feeling better this evening. Now he has a headache, but does not have dizziness, unsteady gait, or feeling like he will pass out. I advised him that headache is a possible side effect of the vaccine, but the other symptoms are uncommon. I asked him to seek medical attention if the symptoms come back. He voiced understanding.

Patient complains of extended experience of pain, swelling, and redness in left unilateral axilla. Pain is significant enough to cause loss of sleep and significant alteration in daily lifestyle. Pain is described as most significant at site of axilla.

Pt was having anxiety after vaccine and was having little shakiness/light head. He was fine in few minutes. He said he always had fear with needles and anxiety in past. Mother was there with him all the time. He was feeling fine in few minutes. Gave him some ice packs and his feet were elevated. I asked him if he/mother wants us to call paramedics, but they denied. He was able to talk - he said it's just his anxiety, he is feeling fine otherwise. Pt left the store after few minutes with mother.

Chills, Restlessness, Nausea, Headaches, Shortness of breath, Dizziness, Muscle and joint pain, chest pain

He had the vaccine on 05/12/2021. It started at 1 am, he had a pretty normal night, he went to bed and woke up vomiting, that continued through the night, the vomiting continued, he had a head ache, he called me and told me that his stomach really hurt, I told him to check his blood sugar, he also had hot flashes, chills, and body aches, but mainly he had vomiting, then the next day 5/14/2021 he was getting more sleepy and less responsive, his friends tried to give him water and different things, his blood sugar went up, his friends took him that afternoon to the ER because he started not responding to me, his friends would tell me he was sleeping, so they took him to the ER, they found that he was very dehydrated, they were giving him fluids and did blood work, he was very unresponsive when he arrived, he could not answer his questions, he had a friend that was there that told them he had the vaccine, the blood work said he had a very high white blood cell count they were worried he had sepsis, they found his kidneys were not working ok, because his levels were very crazy and chest pain and severe stomach pain, and he had pancreatitis, they ruled out a heart attack, he then was admitted because he was in DKA, diabetic keto acidosis, like a result of his type 1 diabetes that got flared up because of all his reactions. They admitted him to the ICU and he was there until 05/16/2021 and basically they were just giving him fluids, potassium and other things to help his body calm down and stabilize his blood sugar. He was released and he still continues that his heart hurts but doctors say that these things resolve after a while.

Cardiac arrhythmia, tingling in head, brain fog, dysautonomia, light sensitivity, tachycardia with any exertion.

My son got the second vaccine shot at work and he works I don't know the lot number or any of those types of numbers

Vaccine was mixed/prepared/given proper protocol. Patient sat down after vaccination and within 5-10 minutes he fainted face forward from a seated position. He regained consciousness within 30 seconds of fainting. He then vomited within minutes after he sat up after fainting. A nurse practitioner was in store shopping and stopped by to read his blood pressure (135/76) then the EMT arrived within a minute of the blood pressure reading.

I administered patient's first dose of Moderna vaccine. While waiting the 15 minutes, patient started feeling funny and got very pale. His mom alerted us and we went outside the pharmacy, put the patient's feet up on another chair, administered 50 mg of liquid diphenhydramine, and got the patient an orange juice. The patient's color started to return and he waited another 15 minutes. The patient stated that he hadn't eaten much that day and that he has experienced this type of reaction with other vaccinations. I called the patient's mom later that day to check on the patient and she stated that he was feeling back to normal.

about 10 minutes into his waiting period, I noticed he laid his head on his dad's shoulder. I went to check on him and he said he did not feel right. he described feeling dizzy and trouble seeing. I got him water, ice pack and took his blood pressure and used a pulse oximeter. His BP was 90/50 and hr was in the 50's o2 were perfect. we continued to monitor his and told him to drink more water. (he consumed 2 small bottles) before he left (30 minutes later) he was feeling much better. His dad said he was very nervous and has done this before with injections.

Just finished administering the Moderna vaccine and whilst still having a conversation with the patient I was about to start the clock on the timer to monitor the patient when his eyes rolled over, the patient slumped in his chair and fell to the floor. He jerked twice then his eyes opened and he looked very confused trying to figure out where he was. I let him stay on the floor for about two minutes and then I helped him get up and sit back into his seat. I monitored him for 30 minutes. There was no difficulty in breathing, shortness of breath or tightening of the airways. No swelling of the face or other sites of the body where observed. The patient was able to carry out a normal conversation. Once the 30 minutes where I released the patient and advised him to get in touch with his primary care physician, Dr.

Symptoms first started 3 weeks after the first injection. Symptoms are best described as mild-moderate and constant lightheadedness/vertigo/dizziness. They came on slowly the first week and escalated that by the end of the second week to the point where I went to the emergency room. EKG and all bloodwork came back normal, I have prescribed meclizine which had no effect. Symptoms vary slowly improved over the course of the next 2 months, at which point progress stagnated. Still feel light-headed constantly, have difficulty focusing, and have felt nauseous in the past, but that has improved. Made worse by caffeine and alcohol. Benadryl and Sudafed seem to have helped in the past, but only for a few days before their effects wear off.

The patient fainted immediately after receiving the vaccine. The patient was fine within a few minutes. Patient was sweaty and anxious for about 10 minutes total. Patient remained in immunization room for a total of 30 minutes and was fine before he left. Patient has previously fainted when receiving other vaccines as note on his informed consent paperwork and as discussed with pharmacist.

7/19 patient presented to ER and was admitted for generalized edema, elevated BNP, tachycardia, DM, and anxiety. Acute CHF, cardiomyopathy, and apical mural thrombus. Patient was diuresed, placed on heparin drip and anticoagulants, insulin adjusted. Echo performed. Labs.

Moderna COVID-19 Vaccine EUA adverse event started as localized myalgia around injection site started around 4:30pm same day as the injection then progressed into a headache and fever of 101 by 11pm that night. Myalgia spread to the entire body and interrupted sleep at 2am. Heart rate was 110 while lying in bed. Took Nyquil for fever and at 1pm on 01/08/2021 fever is down to 99.1.

per report via email - patient receive COVID vaccine during staff clinic in the AM. During the evening patient started to feel sick. symptoms included: nausea/vomiting, uncontrollable jerk like seizures, very disoriented, respirations ragged and rapid. Family called 911 taken via ambulance to local ER. per report patient received IV fluids, medicine to treat nausea/vomiting and meds to "get his respirations under control". patient sent home and is now resting.

Fever, chills/shivering, headache, nausea, difficulty breathing, myalgia, weakness, restlessness Pain at injection site beginning two hours after injection. All other symptoms experienced during the evening, night, and morning after injection. Sudden onset, lasting 12 hours. Constant. Acetaminophen and ice used to alleviate symptoms.

Patient was seen in my office on 1/15/21 due to a reaction from the covid. Reports that "3 of the reserve doctors were running a covid vaccine clinic and were giving the injections themselves." He reports that he "received the first dose/shot in his left arm then about an hour later received the second dose/shot in his right arm." This happened on Saturday 1/9/21 in the evening and he reports his symptoms started late Sunday night/early Monday morning and were significant by later in the day Monday 1/11/21; he reports he had severe headaches, dizziness, nausea/vomiting, numbness and tingling in both arms from injections sites to forearms, "felt like he was going to die." He is unsure if he had a fever because he did not check it. He reports his symptoms improved on Wednesday and "were pretty much gone by Thursday." At his appointment on with me on Friday 1/15/21 he had no symptoms and his physical exam was normal. No injection site irritation and normal neuro exam. He reports he "notified the reserve doctors about his reaction and symptoms and was told at that time that he was given the vaccine incorrectly because both doses were given at once instead of a few weeks apart." He reports his comrades that also had the vaccine given incorrectly on Saturday "were also really sick" but states that "by Sunday the doctors realized their mistake and were giving the doses correctly just one at a time."

Chills, headache, fatigue, brain fog, difficulty sleeping. Started approximately 12 hours after injection

I obtained my booster shot on my left deltoid at 2:30 PM 2/4/2021. I had slight soreness in my arm til 8:00 which the soreness disappeared. Around 2:00 AM, I was awakened by a pounding headache, fever of 102, and my upper left chest (unilateral between the midline of my left chest and the axillary region) started to hurt. I took Tylenol 1000 mg at 3 and took a hot bath (recommended by friend who also displayed a fever and headache). After a while, my fever began to lower but my headache and chest pain did not disappear and I could not sleep until 7 AM. I woke up today (2/5/2021) at 10 AM with a fever of 101.4, another pounding headache, and still unilateral chest pain. I took another 1000 mg of Tylenol around 10:30 AM and it only helped with the fever. I went back to sleep and slept until 4 PM. I understand that the fever and headache are normal symptoms but my chest pain does not seem like muscle soreness. It doesn't hurt when I touch it, it comes and goes every few seconds, I have 4 or 5/ 10 on the pain scale for most of the pain but it turns into 6 or 7/10 occasionally (10 being the worst pain). What does help with my pain when it is 6 or 7/10 is when I clench my chest really hard like I am flexing it. It is a sharp pain that stays for a few seconds before it disappears and it feels like a deep pain. I didn't want to take Tylenol again because I am about to max out (can only take 2 more capsules until 2:00 AM because it states not to take more than 6 capsules in 24 hours, and each capsule is 500 mg) (Also, the Tylenol does not help with the pain so I am only taking it for my fever.) I was wondering if anyone else reported this and if this is an adverse side effect. This has not cause any issues regarding my breathing but just pain.

I almost fainted the night after my shot. I had vaped cannabis prior at around 1:00am, and probably re-dosed at around 3:00AM, before falling asleep at around 4:30AM. I had a harder than normal time falling asleep that night, and I didn't sleep very well. At around 6:00AM I woke up and walked to the bathroom, feeling pain in my anal region. As I started to sweat profusely, I recognized that I was likely about to faint, so I pre-emptively lied down on the floor of the bathroom until I felt well enough to stand up. I don't remember exactly how long I was on the floor but most likely a few minutes. I then had a bowel movement, still feeling pain in my anal region but to a lesser degree, and then drank some water and went back to sleep at around 6:30AM.

"Moderna" Adverse vaccine reaction possible as began 12 hours post second COVID vaccine dose on Friday. A ER note was generated in a T-Con on a patient who got their 2nd Covid Vaccine and began experiencing chest pain approx. 12 hours post vaccine. Patient did not report to the ER until 28Feb21. It was discussed that he has myocarditis v pericarditis, elevated D-Dimer of 770, swollen lymph nodes, and elevated Troponin. Patient was transferred to Research Medical Center due to no available beds. Patient was referred to a Case Manager and Cardiology. The patient completed a follow up appointment with 509 MDG on 9Mar21 and reported Dull pain that has been affecting sleep. Cardiology reports shows Troponin was elevated to 9.1 decreased to 4.2 and back up again to 13.4. Cardiac revealed late gadolinium enhancement, possibly representing myocarditis and then at discharge possibly myocarditis.

Our Son is typically a happy person and enjoys everything around him and everyone enjoys him, he usually helps with everything involving his care. He uses an IPAD for communication, he understands most direction given if it is something he has learned. Since receiving the second dose of the Moderna vaccination our Son has shown bouts of rage, a rash between his shoulder blades and on his left hip area, both rashes come and go and both rashes are the same type and size. He is typically active; however, the energy level is through the roof, what we mean by through the roof, it is so very extreme that he cannot settle down for anything, he normally sits on our couch and watches a movie but he cannot sit for even 2 minutes without trying to throw himself off the couch. It is typical for him to sleep through the night, but now he cannot sleep more then 3 or 4 hours and he does not go back to sleep, he cries which is not typical.

My left arm was sore starting a few minutes after the vaccine. This soreness last through that night. When I woke up the next day, my arm was extremely sore and I was very dizzy. Additionally, my pulse felt fast but my heartbeat seemed to have less strength. My breathing felt more shallow like I wasn't getting as much oxygen from the air that I breathed in as I should. For the first two weeks, the dizziness was very strong. Now, at the three-week mark, it is more manageable and life is somewhat returning to normal. I visited an ENT doctor and their diagnosis was that my dizziness was not being caused by a preexisting medical condition. Their prediction was that I would be fine, though looking back, I downplayed the severity of my vertigo. At its worst, I felt that, when I went for a walk outside, the surface was the Earth was violently spinning through space, with me stuck to the surface. It felt paralyzing. My body felt almost stuck in shock. I was afraid I would be stuck with this dizziness for the rest of my life since it was so long-lasting. Now, I am taking ginger pills and am watching my sodium intake. I feel better but am extra vigilant about taking ginger and not stressing myself too much. I do not plan on getting a second shot since some of my symptoms mirror those of anaphylaxis.

Patient received vaccine and was waiting by clinic for the 15 minute wait time. During the first 10 minutes, Patient started to feel dizzy. Brought patient into exam room to have him lay down. Checked oxygen status and it was WNL. Gave patient some water and patient started to feel better. Mom was at bedside with patient and stated the patient has anxiety and this has happened before with some vaccines/illnesses. Patient was able to walk out of the clinic and no longer felt dizzy.

Pt Fainted shortly after receiving vaccination. Pt came for a moderna covid vaccine with his friend, both first doses. Pt said he "fainted recently" from a shot where he fainted "for a couple minutes" according to his nurse, and he was sure it was psychologically induced by anxiety. He said he was more mentally prepared this time. After I gave the dose to him and his friend, he got lightheaded and slowly started losing consciousness in his seat after a few minutes. Pt was alert and responded to our questions. I quickly stepped out to give the pt a bottle of water, and upon coming back to the IMZ room, his friend said he urinated himself. He was less responsive, but still alert. I notified the technician. He started to turn pale and felt more cold. He pulse was weak. I encouraged the pt's friend to keep him alert while we call EMS. I reminded the pt to keep breathing. The technician called EMS, and I stayed with the patient to kept him calm, but alert until EMS arrived. Pt slowly felt better after a few minutes (color return, and temperature normalized). Pt said he was okay, and he was able to walk. EMS arrived and tested blood sugar; he was within normal range. Pt did not need EMS service, and went home with friend.

Weakness, chills, sweating, nausea, dizziness, headache, restlessness, pain around injection site, joint pain, lower back pain

At the pre-counsel stage, the patient said that he was a little nervous about getting this immunization, but wanted to go ahead and do it. I counseled him on the immunization and sent him into the booth to get the shot. Technician administered the vaccine and then told Technician that he felt a little nauseous. Technician told him to stay seated. A few seconds later the patient passed out and started slipping down the chair. Technician grabbed him from behind and called me over to assist. I yelled for Staff to call 9-1-1 and a code white. I held him from the front and Technician held him from the back and we slowly lowered the patient to the floor. Technician sat behind the patient to hold him up while I assessed the patient and tried see if he was responsive. The patient was breathing and had a pulse, so I patted him on the face and chest to see if he would become responsive. After about 15 seconds, the patient became responsive and confused. He asked what happened and if he got the shot yet or not. After talking to the patient for a moment he said he was fine, but needed to vomit. I gave him a trash can and he vomited. We gave the patient some water and sat with him on the ground for a few minutes until the paramedics arrived. The paramedics took the patient's vitals and the patient said he felt fine and declined going to the hospital. We had the patient sit in the observation area for another 30 minutes. He said he felt totally fine and was ready to go home.

Patient presented to Health Center after receiving COVID-19 vaccine at Health Center, and going back to work site. He stated when he got to work, he began feeling hot, dizzy, nauseous, and pruritic. Vitals stable. He denied chest pain, difficulty breathing. Pt given IM Benadryl 12.5 mg in office and oral hydration w/ water. He was monitored for about 30 minutes, and said pruritus had resolved, but he felt tired. EMS called to transport pt to ED. Pt called by Health Center staff about 2 hours later to see how he was doing. He stated he was on his way home as he had been discharged from ED and told to monitor symptoms.

Oh ok I wrote this earlier but I?ll write it again I got my vaccine at 1:15 I got there early so they have it to me then as opposed to the 2:15 appointment I felt a tightness/ lump in the throat as soon as I got it but I could talk fine and everything was fine as I was taking to people around me I left them came back at 2:15 the person in the pharmacy I had a reaction he gave me a Benadryl and I get fine and he kept me around a bit longer I?m sure he filled the information as he asked for my info and everything then at 4:57 I got some small hives on my right arm not the arm I got it on as I got it on my left arm which went away in 20mins then I went to my local hospital just to be safe and it went away mostly when I got there around 5ish I think? Then they gave me prednisone and reported it as a drug allergy I also got se. wry small hives on Thursday on my hand which went away soon after it came and I?m really hoping I can get a second dose and be fully vaccinated as it was a severe reaction

After vaccination went to observation area and after two to three minutes became light headed. Remained light headed for one hour. Felt like he was, "Mildly high." Staff responded and provided assistance. VS WNL.

Vaccine was administered at approximately 6:15PM and patient was instructed to sit in waiting room for 10 to 15 minutes to make sure he was OK. He looked fine and sat looking at his phone for approximately 5 minutes. He then stood and walked to pick up window and asked if it was normal to have stomach pains. I said no and asked if he felt dizzy. He said "no" then turned to go back to his chair. As soon as he turned he said "now I feel dizzy". I told him to return to his chair and he suddenly crumbled to the floor. I had my technician call 911. After 1 to 2 seconds of being unresponsive he suddenly came to. I had him continue laying on floor for a couple minutes and then he got up on his own and went to the chair. We gave him a cool towel for his neck and had him lean his head forward and down onto a chair placed in front on him. The paramedics came within minutes and found him to be OK. After they left he continued sitting in the chair for approximately 10 more minutes while talking on the phone (to his parents, I think). He then left stating that he was now OK and that it probably happened because he was nervous

He consented to transitioning to lying down on a couch and hydrating with a water bottle. BP was within normal limits. He did not faint. After ~15 minutes of lying down he felt recovered and normal. Patient was able to walk out fine with his friend.

Received oxygen via nasal cannula + Nonrebreather, Was instructed by Dr. to visit a different ER to test for blood clots. Multiple PTSD episodes in Hospital during Hospitalization.

Pt received 1st covid shot in left arm. Became wheezy and described feeling like he was going to pass out. Paramedics called to store. They observed pt and determined reaction was not from a vaccine ingredient but instead a panic attack.

First nausea and headache at around 6:00pm until days later. Then shortness of breathe once around 10:30. Near 11:00pm I started getting fever, chills, trouble staying asleep for more than 30 minutes. Loss of appetite for the following days. Felt like worst case of flu ever. Symptoms stayed from that Friday up until about 6:00pm on Sunday night the 25th.

Patent received the shot, was given a 15 minute timer until it was safe for him to leave, He went out and sat in the waiting room until his time was up. Approx, 10 minutes later I heard a noise and looked over at his and noticed he was started to fall out of his chair. He then became as stiff as a board and I held him in place until he came to. I woke up at 9 am with a splitting headache, tried to stand up and I was dizzy and more sore than I had ever been across my whole body. It was incredibly hard to walk also, but I would contribute alot of that to the adductor exercise I had done the previous day. I was also drained of enegery and just wanted to sleep though no position was comfortable. I had an appetite and was able to eat and take some acetaminophen. This along with rest, alot of water and patience seemed to work. The side effects from the vaccine were gone by the next morning.

Situation: 21M had an appointment for 2nd dose Moderna (Lot# 007C21A) today. Shortly after being vaccinated, patient started to feel dizzy and eventually complained of chest pain. Background: Patient has NKDA and allergic to pollen and dogs. Denies PMH. The Moderna vaccine was administered at 11:56AM. Pt came to the observation area at 1205. Pt started to feel dizzy and told girlfriend about it. Juice and crackers provided. Pt consumed 2 juice boxes and crackers. Pt felt better initially then started to feel dizzy and shaky again. Assisted to gurney and took to room 7. Assessment: 1215 VS 98.8F, 127/77 (94), 62, 98% RA, 16 and dizzy. Continue to give more juice and water. Pt started to feel better. 1220 Pt looked better, feeling better, conversing to girlfriend, and playing with phone. Manager at bedside talking to pt and gf. Pt stated that he didn?t eat nor drink today. Pt woke up and went straight to the vaccination clinic. Pt does have hx of anxiety episodes. Girlfriend also reported that patient became dizzy after first dose of Moderna but did not disclose to anyone.

About an hour after getting the shot, the skin on my right arm was flushing before going numb. Then my shoulder went partially numb and it's still numb/frozen as of 2:20 am 26/June/2021. Over the course of nine hours from when I got the shot, starting at around 1 hour after, I started to get symptoms, which as of right now are still here. After my skin started acting up, my blood pressure dropped and my heart was pounding extremely hard in my chest and felt tight. My muscles have been going between losing muscle tone and being locked up extremely hard. I've been dizzy since about 3:00 pm 25/June/21, and it has progressively gotten worse to where I feel like I am going to pass out if I stand up. I have chills that make my chest tighten up and cause angina. I have been feeling very faint since around 9:00pm. I've had a mild headache since around 2:30pm, and have been feeling like I am falling since about 1am. Breathing takes manual effort. I have been feeling dissociated and nauseous since about 6pm and cannot remember much from after 6pm. I am not sure whether it is a medical emergency or not but I can't afford to go to the er.

History: pt had 2nd Moderna COVID-19 vaccine yesterday at 10am and since yesterday evening, pt has had chills, headache, dizziness, low back pain, and nausea. Took Tylenol at 0030. Pt denies runny nose, overt fever, cough, sore throat, vomit, diarrhea, rash, sick contact, visit to Area II or contact with COVID-19 hotspot/cases in last 2 weeks and no co-morbidities.

Systemic: Dizziness / Lightheadness-Mild, Additional Details: patient indicated slight dizziness post vaccination, questioned pateint on feeling nervous, patient indicted that he felt slighly anxious and did not eat all day, gave patient Gatorade, and had him lay down. Patinetts dizzinies resolved after few minutes, and drinking Gatorade. Asked patient if he wanted an ambulance, he indicated that he was feeling normal and did not want an ambulance. Follow up call with patient at end of my shift, patinet indicated that he had a meal and felt perfectly fine.

Blurred vision, difficulty breathing (pale skin/blue lips), profuse sweating, muscle fatigue, headache. This lasted about 15 minutes. Until severity went down. Followed by 20 minutes of profuse sweating and headache. I thought I was going to die

Immediate soar arm, next day horrible body/joint/head aches, intermittent fever up to 103.4 degrees F, mind fog, severe malaise and fatigue, chills. All those lasted about 4 days. They really bad ?COVID toes? that were like chlybains and now 3 weeks later look like petechiae on top of toes (reddish - purple , a few are bleeding). By the way I had Mono 13 months ago that was pretty bad and had COVID in November not that severe at all.

Noted to be diaphoretic, stated had dizziness with nausea which subsequently resulted in vomiting x2. VS: 98%, 58, 102/44. Stated that he had only 2 hours of sleep today, started work at 0230 am. 1232: Stated feeling less nausea, continued to observe. VS 99%, 67, 106/58. 1243: 98%, 69, 16. Stated feeling much better, no longer has dizziness, no nausea, no diaphoresis. 1253: 99%, 63, 108/54, stated continues to feel better, no nausea, no dizziness and would like to leave, left on own recognizance.

Injection arm soreness worsening throughout the day. Fatigue worsening throughout the day. Headaches. Body soreness & tiredness. I almost feel like I am in a bit of a fog too.

PT DISCOVERED INATTENTIVE AN SQUINTING HIS EYES. ONCE ALERT, PT C/O OF LIGHT HEADEDNESS, SWEATING, BLURRED VISION, PARESTHESIAS IN BOTH ARMS AND LEGS. VS @ 3:08 BP 78/62 HR 86 REPEAT VS @ 3:20 PT GIVEN GATORADE FOR REHYDRATION AND INSTRUCTED TO BREATH SLOW LY AND DEEPLY. AFTER 10 MINUTES, PT SYMPTOMS STARTED TO RESOLVE. SYMPTOMS COMPLETELY RESOLVED WITH TIME. PT BECAME STABLE TO LEAVE OBSERVATION SITE. INSTRUCTED TO F/U WITH PCP.

Starting on December 24 and lasting until March 1, I experienced the worst insomnia I had ever experienced, as well as depression, anxiety, and paranoia. Previously, I was doing well. My parents and I saw a night and day difference. I was also throwing up regularly. I also became much more angry and moody. I often felt sick to my stomach.

Within 3 minutes of being escorted to our waiting area and starting the timer, patient made a complaint of sweating. within moments patient was heavily perspiring and lost all color. Flushed and confused. Pt starting asking "how long have I been here?". then while being escorted back to our vaccine room patient experienced a brief syncope. He fainted and was escorted to the ground. Increased pulse and continued confusion. Patient was placed in supine position with ice packs on face and nape of neck. EMS was called. we stayed with patient until EMS arrived and took over. Patient declined going with them to the hospital after his evaluation and was cleared by EMS to leave.

NO ADR. Patient was anxious due to needle. He had his vaccine and was asked to sit on chair for 15 minutes. After sitting for 5 minutes, he suddenly fell down from the chair, lying on floor head down. He was anxious and a bit shaky, but conscious of surrounding. He was talking while lying down. Patient refused to call 911 and did not wanted any help. Patient's father was shopping in store and was called to pharmacy. Patient laid down on floor for few minutes and then sat back on chair. After 10 minutes, he was perfectly fine and left the site with his father.

After the shot was given, he seemed nervous and I asked if he was okay and he said yes. So I turned around to get a bandaid and he fell off the chair. He was conscious the whole time but didn't remember if he got the shot. 911 was called and I observed him while we waited for them to get here, he was never taken to the hospital. Paramedics observed and released him . Patient said he always gets nervous when he gets any type of shot or blood drawn and this type of situation has happened in the past. This was unknown to us when administering the vaccine.

Mom described that patient rec'd second dose in the PM of 03/30/2021. The next day, he was on virtual classrooms and some classmates pointed out to him that they were having to repeat information they had just gone over. After this happening a few times, he became concerned and discussed with his mom. They then realized he had no memory of getting the second dose, or dinner or any other events from the remainder of 03/30. Mom called MD, who advised they go to ER.

Patient got moderna vaccine and he sat down on the chair, and after 5-6 min later he fell from the chair on ground and lie down completely. Pharmacy Team rushed and confirmed he was okay, and after min later he was all fine, gave him water to drink, and check his blood pressure and pulse (118/80 and pulse 85), seems like it he had panic attack after talking with him, he stated that he hates needle and he was all nervous about entire process, but after 20 min sat on the chair, he was all okay, did not need to call EMS, He was able to communicate well and walk well. called patient around 6pm and he is doing well at home. NO concerns.

I got the Covid-19 vaccine on Tuesday April 1st and then 2 days later on Thursday April 3rd I started developing headaches and they got worse and I would say they fluctuated to migraines. Every day I would have a degree of a migraine (which I do NOT have history of) and most days the head pain limited my activity significantly. I had 2 evenings where it got really bad. Once on Saturday April 3rd and then again on Wednesday April 7th. I have had headaches continueally since then. Some days better than others and some days worse.

c/o mild numbness to the throat and anxiety. Denies chest pain, SOB, swollen tongue, or difficulty swallowing. Vital signs : BP: 140/98 HR: 77 Sp02: 98% room air Refusal form signed for ER transfer. Patient states symptom is mild. advised to go to ER if symptom worsen, verbalized understanding seen and cleared by EMS

I started to salivate a uncontrollably for hours on April 14, at around 1am. Still happens now sporadically. I had nerve damage to my tongue from oral surgery so I dont know if its related. I started to develop vertigo today the 15th around 1pm. Vertigo has come and gone on waves. Have had a mild headache all day as well. In addition Ive had alot of trouble concentrating.

Patient complained of dull chest pain with pain score 3 of 10. No radiation. BP 124/80, SpO2 98h, HR 74 and respirations 18, awake and oriented x3. Evaluated by Internist, EKG ordered with results meeting criteria. Likely underlying panic attack. EMS activated and taken to the ED. Called 4/15 but no answer. Mailbox full and unable to leave message

Patient fainted shortly after receiving second dose of Moderna Covid vaccine. When I approached the patient he was laying on the ground and very pale but still breathing. I rolled him onto his back and elevated his feet and asked him to keep breathing and try to stay calm. I asked someone to grab him some water and someone else to run to the pharmacy and grab the pharmacy manager while I monitored the patient. I asked if I could help him sit up to drink some water and he accepted. After sitting up I asked if he had eaten and if I could get him something to eat. He stated he hadn't eaten all day and that he had a panic disorder and felt like he was having a panic attack. I asked if he had medicine he needed to take to help him feel better, he stated no, he needed to just wait it out. Then he told me he felt like he was going to throw up and I got him a waste basket that he threw up in right away. I asked the patient if he hit his head or if he wanted us to call an ambulance; he stated no to both questions. The pharmacy manager arrived and sat with the patient for the next half hour or more until he felt better. He stated that his dad was going to take him home now that he felt better. Some store staff had us fill out an incident report and the pharmacy manager walked the customer out to his car where he said he would call and find his dad.

Around 3 pm on 4/14/21, I started to feel very tired and I started to get a headache. Around 6 pm, I started to experience lower back pain and my legs began to throb and my whole body became very sensitive to touch. I took tylenol or ibuprofen every 4 hours for the rest of the night. This went on until Thursday 4/15/21, when the dizziness and throbbing headache started. Every time I would stand up I would become off balanced and need to hold on to something. I was still experiencing the lower back pain, leg throbbing and skin sensitivity but the dizziness and headache became much more intense. Around 9:30 pm on Thursday, I felt like I was hallucinating and was experiencing euphoria along with a fever. I did not sleep the night of 4/15 into 4/16. I was sweating profusely with a throbbing headache that would cause spinning when I would stand up. Around 3 am on 4/16, I fell asleep until around 4:30 am when I woke up in a pool of my own sweat. The body aches, leg pain and skin sensitivity went away along with my fever. Throughout Friday 4/16, I had a throbbing headache and was dizzy with no relief. This continued into Sunday 4/18 when I started to feel some relief. My headache was pretty much gone but the dizziness is still very much there. I have also an experienced elevated heart rate and respirations throughout the last 5 days.

Approx 15 minutes after receiveing 2nd dose of Moderna, patient reported feeling a "lump" in his throat. Symptoms progressed to light-headedness. Pt was administered epi and benadryl and transported by EMS to nearest emergency department.

Symptoms: Extreme pain immediately after injection at site, closely followed by racing heart and winded and dizzy, followed by sweating, blurred vision and confusion. Don't remember hitting horn to alert officials. Got out of car, dizzy and stumbled. Blacked out and fell to pavement. Don't remember falling, woke up with extreme mind cloudiness, drowsiness, and hands were numb to touch but felt extreme pressure on palms. When pressure subsided, finger tips felt inflamed.

~6 hours after shot started to feel body pain and headache. Got worse and added in fever, weakness and nausea. Was the worst 9-12 hours after the shot. For the rest of the next day I still felt bad but condition slowly improved. Took Tylenol a few times which helped a bit

Patient reported excessive sweating, mild confusion, dizziness. Advised to self monitor and seek medical attention if worsening, may treat with APAP, fluids, rest, and diphenhydramine.

Pt felt nauseous, hot, started getting red, and sat down on floor about 5 minutes after receiving the vaccine. I was alerted and I helped him into the vaccination room to lay down on the floor with his feet propped up. His girlfriend was there to help calm him down. I offered to call 911, he refused. I grabbed a fan for him, and turned it off when he requested. He then began shivering rather uncontrollably and said his left arm (injection site) was hurting and tingling, and at one point, said his right arm was beginning to tingle as well. We had him breath and relax, he began to return to normal and he and girlfriend discussed how he must be partly having an anxiety/panic attack. He trialed standing up a few times until he felt good enough to leave the room, all in all about 20 minutes. He sat close by for additional 15 minutes of observation, at which point he left stating he felt more like himself.

Approximately 5-10 minutes after receiving injection patient became faint and collapsed while browsing store. Did not appear to hit his head or significantly injure himself in any way but reported headache and some confusion on regaining consciousness. Slowly moved to sitting position and allowed to recover.

Tightness in chest, pain in chest, and shortness of breath. Started late May 13, subsided enough to get some sleep. Awakened by the pain and shortness of breath around 7:30 am May 14. Unable to get out of bed. Called 911 for ambulance. Patient was taken to Hospital Cardiac. EKG showed ST elevation, thus L & R Catheterization was performed. Negative for MI, but diagnosed with Pericarditis, kept overnight in hospital and prescribed Indomethacin (Indocin) 3 times daily for 14 days. Released from hospital May 15. Recovery continues at home.

my son had swelling in his throat and mouth , confusion and mild delerium after the second dose , fever of 101-103. vomiting , Muscle aches , migraine headache

It started off with me becoming really irritable over the smallest things. I started getting cold chills even though it was 70F. I had a nightmare that felt like an eternity. My dreams get warped when I'm sick. I woke up and I had a headache that was centered around several spots in my head. I couldn't get comfortable no matter what I did. It's impossible to lay down on the side you get the shot on. It hurt like... 7x more than the first one when they're administering it. The scariest symptoms though, for me, was that I couldn't catch my breath. You know when you take a hot shower and you can't really breathe 100% because of the steam? That's how I felt all day after the 2nd dose. Also, I couldn't tell if I was full or not when I ate. The only way I could tell was stomach pains. That's all that's happening/has happened to me. I do feel better alot better though two days after having the 2nd dose.

Symptoms much like a concussion however not suffuring any head injuries. Strong persistent headaches after long periods of complex thoughts of new content. Causes need to pause for long periods of times before continuing any learning. Being upright for long periods of time may also cause this effect from observations at school. Difficult or near impossible to retain any and all new information. Recall or use of previous experiences or knowledge shows without a severe headache. Sitting down for long periods of time and working from there mitigate a fair bit of the strain. Anatomy teacher had thoughts it may be an increase in cranium pressure due to the throbing in the back of head. Changed just about anything in my daily schedule to try to isolate the variable by changing one thing a day, eventually got through my day to day with no potential variables isolated. (On going) Notable less headaches with work i am familiar with or have a deep grasp of already. Tried picking up other hobbies during the pandemic with no luck in retention.

After recieving the shot, I waited the 15 minutes and felt okay then. Maybe a little nervous thinking I COULD possibley be unlucky few and have a bad adverse effect. However on the drive back home, calmly singing along to my music, I all of a sudden felt an increase of heart rate. I could still drive fine, but my head had a radiating feeling and it's almost like I got a random adrenaline rush. My heart rate must've been 150+ bpm. It lasted for 20ish seconds and started to go away as I slowed my breathing. Not sure if it was a direct effect of the vaccine or my mind...as I had never gotten such effect on me.

I was sitting calmly as weekend classes were ending. I experienced a rapid heart rate followed by numbness in hands, legs and eventually face. I couldn't walk or breathe. I was transported to the hospital by ambulance and was told by the paramedics that I was having a panic attack.

Went to the hospital after passing out 3 times a few hours after receiving the 2nd dose. Then experienced rapid heart rate and anxiety episodes reoccurring on and off for 3 weeks after receiving the vaccine.

Pt was receiving 1st dose of Moderna vaccine. I noticed he seemed very tense and nervous. I had difficulty getting him to relax his arm. Seconds within giving him the shot he fell forward hitting his head on the ground. He came to within a few seconds with no recollection of what happened. I gave him some water and had him sit down. I asked if he had any previous reactions to a vaccine, he said he hasn't but he does get lgih

Since getting the vaccine, I have developed multiple strange health conditions. The first is that I have been nauseous and usually vomiting every day for at least the past 2 to 3 months. Also since getting the vaccine I have developed nerve pain and paresthesia in my hands that does not get better with rest and that I am now being treated for because it is so bad that I cannot work on my computer anymore. I also can no longer eat gluten without severe gastrointestinal distress, despite the fact that I have never had issues with gluten in the past. Finally, I have also developed two strange growths on both of my feet in the same spot on both feet. My primary care doctor thinks that they are ganglion cysts and are benign, but I thought it strange that I would develop the same thing on both of my feet in the same spot at the same time.

patient received moderna vaccine at 12:15. at approx. 12:20 patient stated that he "felt weird" patient appeared pale, diaphoretic, and complained of losing hearing and blurred vision. we sat the patient down and took his vital signs and called 911 at 12:20. Patients blood pressure was 88/42, heartrate was 55 BPM, SPo2: 99%. Patient never lost consciousness and was talking to us throughout the entire event. Retook vital signs at 12:22 BP: 88/55 HR: 53 Retook vitals at 12:25 to BP :94/54 HR: 57 O2: 99%. At 12:30 Ems arrived, and assessed patient, patients mother was called at 12:25 to notify of event, patient declined going to the hospital. Patient stated he would wait for mother to arrive and leave with her. Mother arrived and drove patient home at 1:00 pm Patient stated before leaving HD that he didn't eat any food today but he felt a lot better.

23 yo male received COVID-19 Moderna vaccine at 11:04am , patient monitored x15 minutes, no complaint or adverse reactions noted, discharged at 11:19pm Patient returned at 11:30am after going to his car, patient stated "I went to my car and took a sip of coffee, I felt like there was something in my throat". Monitored Patient x 45 minutes, patient alert and oriented x 4, calm, skin color normal, no rash, BP 125/86, P 84, pulse ox 98, no complaint of worsening or change in condition. Spoke with patient's father x 2, requested father to pick up son, but father felt not necessary. Notified medical advisor, recommended discharge to immediate care or ED via self. Parent agreed with self transport to ED after speaking with patient. 911 transport refused by patient. Safety instructions reviewed with parent and patient, both verbalized understanding. Patient discharged to transport self to Emergency Room, parent aware.

Early on in the day of 1/6/21, I felt very sluggish and spaced. My boss asked me early in the day if I was okay because she said that I seemed "out of it." It wasn't to the point that I was unable to work, but it was a noticeable difference from my typical state. At 11:31 AM, I texted my wife and said "I feel gross and weird today." When she asked me how so, I replied, "I just feel run down, my neck hurts, and I can't focus." My arm was more sore than it typically is from the flu shot, but it wasn't unbearable. As the day went on, I felt the cloudiness in my head start to dissipate a bit and I started to feel a little better. By dinnertime, I was feeling sick to my stomach and was not able to eat very much. I chose to head to bed around 9 because I was feeling pretty ill (my neck had really started feeling sore again and my stomach was very upset.) At 1:30 AM on 1/8/21, I woke up with a horrible headache. I get migraines from time to time, but this was unlike anything I had ever felt before. It felt like my head was in a vise and I was very close to asking my wife to take me to the ER. I never go to the doctor, but it legitimately scared me. My neck was extremely sore and my head hurt so bad that even laying it on a pillow hurt. I was extremely nauseous as well. I took a few extra strength Tylenol and laid my neck on a heating pad with an ice back on my forehead. The sound of our humidifier was too much for me, so I also out in earplugs. After an hour or so, I fell back asleep. When I woke up this morning, I still had a headache and I took an Excedrin. To this point, I have felt a little sick to my stomach, but the intense headache has been kept at bay. My neck is still sore but it is also somewhat better.

Patient came back from the unit after returning back to work stating that he "felt like he had run a marathon". Upon assessment, patients vital signs were BP 152/110, P 83, O2 sat 99% although patient stated that he felt "short of breath". Patient reported a history of anxiety and HTN, but was not taking medications for blood pressure as he had not "picked them up". Doctor assessed and called EMS. When they arrived, they did ask him if he wanted to go to which he replied "it would probably be best". No further complaints or treatment reported at this time.

Soreness in arm at injection sight. Muscular pain and general fogginess of thinking for the next 24 hours with intermittent migraines.

Woke up with a headache and fatigue, as well as soreness near area of injection. Energy was very low and felt very tired. Tried to sleep at around 12:00 PM but had a thumping in my head that would not let me fall asleep. Around 3:00 PM tried to sleep again but being shivering/shaking a lot despite being under a blanket and comforter. Eventually needed to take a hot bath for around 45 minutes to an hour. Took my temperature while in the bath which read 96°F (although that thermometer may not be the most accurate.) After the bath I went directly to bed with a comforter, blanket and weighted blanket and was able to fall asleep for about 90 minutes. Was no longer shivering when I awoke and felt much better. Went to sleep at around 9:00 PM and felt fine when I woke up the next morning.

The night I received the second dose, I was unable to sleep and had developed a fever. The next day I had the worst headache of my life and my body kept switching between feeling hot and cold. That night, I was able to sleep, but I woke up drenched in sweat. Day two after vaccination, I had a mild headache.

Patient fainted post injection. He stated that he had anxiety and felt like his blood sugar might have dropped. EMTs checked him over and patient declined treatment.

Nothing at time of injection. In the evening, soreness in the arm (strong but on par for what I would expect). Next morning, I starting feeling sick, with flu symptoms. Aching muscles, sensitive skin, mild headache, sore eyes, and in the evening, nausea. Temperature gradually rose. ~99.7 at 6pm, 100.1 at 7pm, 100.7 at 8pm, 101.7 at 9:30pm. I took some Tylenol around 7pm, and then some theraflu at 10pm. Fell asleep at 11pm. Periodically waking up every 30min from 1am onwards. Muscle discomfort, sweating, and nausea continued until 3:45 AM, at which point I was feeling better and was able to sleep continuously. Feeling much better in the morning on March 4th. Temperature was 99.9, then rose to 100.0 at 11am. Now, at 3pm, feeling fatigue, exhaustion. No headache. Normal sense of taste and smell. No couch. Taking another day off from work. Temperature is currently 100.0. Taking more theraflu now, have not taken Tylenol today.

~2 hours after vaccination, around 1:30 PM on March 5, strong pain and soreness in the left arm started. Later at around 6:30 PM I took an Advil tablet for the pain. At around 7:30 PM I took a second one since the pain was still strong after the first one. I took a third Advil tablet at around 11:30 PM. At around 12:30 AM on March 6, while going to sleep, fever-like symptoms started such as high body temperature, chills, congestion and aches. Fever symptoms continued as I intermittently woke up around 6-8:00 AM, and as I woke up again around 9:50 AM the feverish side effects had mostly subsided. Arm soreness has also decreased dramatically but is still present.

While technician finished administering the vaccine , patient had seizure like reaction that lasted for about 15 seconds . Patient immediately gained consciousness and had no recollection of what happened. He waited for 30 minutes and totally fine. Prior to giving the shot, patient checked the box for feeling dizzy after vaccinations, when I inquired about it , he said that was years back and he had no issues with any vaccines that he has taken. He take flu vaccine every year.

First vaccine on 1/26/21 began taking three naps a day and arm pain. Given Ibuprofen. Seemed to resolve after 5 days, but continued to nap three times a day sometimes. No fever, but hot flashes at times. Booster given 2/23/21. Began to nap again three times a day or want to stay in his dark room. Began to have a lot of sinus drainage. Gagging at times. Pain over right eye area. Pain in right flank and stomach area. Severe headaches and possible body aches. No fever, but increased and more severe hot flashes that would wake him. 2/28 had a tonic clonic seizure after presenting with a distressed look throughout the day. Slept and upon waking began vomiting until 3am. Was at the ER and given many tests, pain meds and nausea meds. Ruled out kidney stones with CT scan and urinalysis. No fever, but hot flashing continued. ER visit lasted from 2/28/21 through 3/1/21 ER Physician suspects possible reaction to vaccine. Followed up with Neurologist and General Practitioner and both suspect reaction to vaccine.

A couple of hours after my fist dose Moderna vaccine, my left arm began to hurt, I also developed headaches, body aches and left arm swelling. At 1am-2am, I could not move my left arm and my fever was 103.3F. I contacted urgent care who told me to take Ibuprofen as pain medication. My symptoms did decrease although I still have today difficulty sleeping and sweat at night which I did not have before the vaccine.

Lightheaded and trouble breathing. Pt was anxious. Had some water, rested with elevated legs and felt better. Ambulated out of facility without assistance

Faint; Migraine; Dizzy; Disoriented; Joint pain; Fatigue; Muscle pain; Nausea; Slight fever; A spontaneous report was received from a consumer concerning a 23 year old, male patient, who experienced faint (syncope), migraine, dizziness, fatigue, disoriented (disorientation), joint pain (arthralgia), muscle pain (myalgia), nausea, and slight fever (pyrexia). The patient's medical history was not provided. Concomitant products known to have been used by the patient, within two weeks prior to the events, included allergy medication unspecified. On unspecified date, the patient received their first of two planned doses of mRNA-1273 for prophylaxis of COVID-19 infection. On 10-FEB-2021, the patient received their second of two planned doses of mRNA-1273 (Lot number: 013L20A) intramuscularly in the left arm for prophylaxis of COVID-19 infection. On 10-FEB-2021, after vaccination, the patient experienced faint, migraine, dizzy, fatigue, disoriented, joint pain, muscle pain, nausea and slight fever. Treatment for the event included Ibuprofen and Excedrin. The patient received both scheduled doses of mRNA-1273 prior to the events; therefore, action taken with the drug in response to the events is not applicable. The event of faint was considered recovered. The outcome for the events of migraine, dizziness, fatigue, disoriented (disorientation), joint pain (arthralgia), muscle pain (myalgia), nausea, and slight fever (pyrexia) was considered unknown.; Reporter's Comments: Based on the current available information and temporal association between the use of the product and the start date of the events, a causal relationship cannot be excluded.

I received the first shot of the Moderna Vaccine at approximately 8am on April 1st, 2021. I had no symptoms or side effects immediately except slight fatigue and a sore arm. I went to bed around 8:30 PM and woke up roughly an hour later with horrible body chills and probable Fever. I have been sitting covered in multiple blankets shivering and unable to sleep.

Systemic: Dizziness / Lightheadness-Severe, Systemic: Hypotension (BP 88/73, HR 91), Amnesia? Did not remember what occured.-Severe, Systemic: Flushed / Sweating-Severe, Additional Details: Pt was layed on floor and feet elevated. BP during incident was 88/73, HR was 91. Pt recovered briefly after, but did not remember what happened.

Pt fell faint. Anxious about vaccine. Did not hurt his head. Landed on his buttocks. No complaint of headache, dizziness, chest pain or shortness of breath. Patient alert, awake, oriented. Vital signs were taken, 11:20 blood pressure 113/82, heart rate 76, spO2 98%.

Severe soreness from the waist down, feeling like body aches as a flu symptom. Exhaustion. Felt plain crappy. Left work early (20 hours after vaccination) and slept most of the afternoon and night. Feel pretty much back to ?normal? this morning (42 hours after vaccination) No medication taken or doctor visit. Just rest.

Pt stated to be a bit nervous about receiving his initial vaccine for covid. He said he sat down at 2:51. He said he did not recall passing out just waking up. Vital signs were taken to know pt baseline. All vital signes were wthin norma limits. He stated to feel a little sweaty and hands be a bit cool. He became more talkative with staff and more contrfitriblwe. Pt left after feeling better and safe to drive home. He was encouraged to bring someone else with him next time.

pt slumped in chair and fainted about 5 minutes after vaccine, was unconscious for about 10 seconds, came to suddenly, very disoriented. Very sweaty and continued to drift in and out of consciousness. No sign of andioedema or swelling of any kind. He was able to communicate mostly, just very drowsy and disoriented. EMT was called and was assessed. He refused going to hospital, remained at store for about 45 minutes after incident. Went home with family still somewhat dazed.

Patient had nausea, dizziness, shivering, trembling, and sweating, BP 101/51, HR76, RR12, T99.1. Symptoms did not resolve after 25 minutes, ED was called and patient taken to ED. Upon arrival to ED, was hyperventilating and had anxiety. ED BP 121/74 HR77 RR20, T98.2F. EKG normal. Monitored in ED, then discharged once stable.

Pt was noted to be sliding/falling our of chair to his right side. Patient appeared unresponsive. Pt was hoisted down to the floor (by this RN) and placed in Trendelenburg position. Pt?s eyes were noted to be open with some snoring respiration sounds. Pt?s facemask was removed. Pt then became arousable, confused about circumstances, and what had happened. Pt reports he believe he fell asleep in the chair. Pt was able to ambulate out of the facility without assistance after an appropriate amount of time. Patients vital signs were recorded during the event.

I woke up in a full sweat and it was cold in the house after waking up had a massive headache with body pain was very hard to wake up to go to work when I did go to work I was in pain and very nauseous was sent home from work and go home lay down and went back to the pain in suffering woke up the next day feeling good

Pt fainted, fell off his chair onto the floor where he was convulsing. He then regained consciousness shortly after. Sweaty, clammy, pale, did not remember what happened. Asked "what happened?". 911 was called, the paramedics came. Took vitals. They administered IV fluids and wheeled him out on a gurney in a neck brace.

Fever, headache, chills, nausea, insomnia. Treatment was time, rest, hydration, and eating enough carbs.

8/10 kidney and lumbar pain starting at 6AM the day after injection, mental fog, overall muscle weakness. Pain treated by moving around until found a position that hurt less. Pain reduced to 5/10 by 11am and 1/10 by 2pm. Tired feeling remains.

Individual stated he was anxious about needles and the vaccine. Within 10 minutes of receiving the vaccine, he stated he was lightheaded, fatigued and slightly nauseated. He did not faint. He was given cold water and we had him lay down. We observed him for 30 minutes after onset of symptoms. He was too fatigued to go back to work. He was escorted home. We checked on him 5 hours after the vaccine. He stated he was still very fatigued but was not lightheaded or nauseated. He was able to eat and drink.

patient was seated in chair in waiting area about 2-3 minutes after vaccine. he started seizing and straightening up his body and began falling out of chair. i ran over and caught him before he fell all the way out. i supported him in the chair for about 10 seconds and he came to. the whole ordeal only lasted about 15 seconds. he said he felt weird and remembered dropping his phone. he stated he did not eat that morning. i gave him water and a snack and we watched him while EMS was called. they showed up in about 5 minutes. took vitals, everything came back normal. pt did not have hx of any disease states. parent picked him up from pharmacy.

Events: chills, aches, headache, insomnia Treatment: none Outcome: most symptoms gone within 24 hours

Syncope, sweating, "feeling out of it"and that he was going to pass out. Had to lie on the floor for at least 20 minutes and even then he feel bad when he would get up, all of the same symptoms.

10:25am feel nauseated Vital signs 122/82 Pulse 67 pulse ox 99%; 10:35am patient reports feeling very anxious and dizziness; 10:50am pt reports chest pain; 911 called; BP 136/86, Pulse 77, Pulse ox 98%. 10:55 am EMS arrives patient monitored, glucose 83; no abnormalities in cardiac monitoring. 11:03 released by EMS - instructed to rest and take deep breaths.

After receiving Moderma vaccine, pt came to 15 minute wait area. once here, pt stated he started getting a headache. The headache stopped , then pt began feeling extremely tired and shaky. Broke out into cold sweat. Never passed out. Pt his arms felt lie they were "asleep" and fingers on his left hands felt like he could not move them, stiffened . No respiratory distress listed VS taken

Fever arm pain 24 hours after second shot 3 days after second shot: numbness, tingling in toes, some numbness, weak knees. Tight muscles These symptoms have continued now in 6th week. Now burning in toes at night waking him up.

Patient did not state any problems prior to getting immunization. Patient stated "just fine" for the few minutes immediately after receiving immunization. When patient attempted to walk he got dizzy and fainted. After event, patient stated he had previously been very anxious and not reported it. He stated he was very anxious and fainted for that reason.

Client received the 1st dose of the COVID vaccine (Moderna Lot # 027C21A and expiration date 07/01/2021) at 1934. At 1948, EMT noticed that the client?s face was becoming pale and diaphoretic. EMT alerted PHN and both responded to the patient at 1948 and transitioned the client into the zero gravity chair. Client was reluctant to sit in the zero gravity chair and stated, ?I?m feeling anxious and this is not helping especially with my aunt.? Client?s aunt was very concerned and was anxious about what was happening and was standing over the client. PHN asked the aunt to sit in her seat and allow us to assess the client. PHN also explained to the client that this was a precaution and not meant to create more anxiety. PHN offered the client a juice which he accepted and drank. At 1952 client stated, ?I?m feeling fine now, I?m not sweating anymore.? Vital signs were taken at 1952 (Blood pressure 130/70, heartrate 87, Oxygen 98%). At 1952, client?s color had returned to his face and his skin was no longer diaphoretic. PHN asked the client if he has history of fainting from needles, and the client stated, ?no, but I have anxiety.? Vital signs were taken at 2002 (blood pressure 112/70, heart rate 68, oxygen 97%). Client stated that he ate two hours before receiving the vaccine. Vital signs taken at 2010 (blood pressure 114/74, heart rate 78, oxygen 97%). Vital signs taken at 2018 (112/74, heart rate 66, oxygen 97%). Client does not have any medical conditions or allergies and is not taking any medications. PHN provided education to the client on when to seek emergency medical services and to follow up with his primary care provider before receiving his second dose. At 2019 client stated, ?I?m feeling fine,? and left the facility unassisted with a steady gait.

Tingling and numbness in left pinky and ring finger, feeling of discomfort in elbow, lower arm like hit funny bone, left hand gets stiff and trembles when trying to extend. Mild symptoms first 2 days, but now significant and unable to type or use left arm without discomfort so have been resting it. Over 3 weeks with no improvement.

Chest pains beginning around 48 hrs after vaccine administration and ending around 2 weeks after receiving vaccine. Shortness of breath at night while sleeping for approximately two weeks after receiving vaccine.

Patient was sitting in observation area immediately post vaccine and he suddenly lost consciousness and his head slumped back briefly. Within 3 minutes patient regained consciousness and was very sweaty and anxious. Provided patient cod compress and water and blood sugar boosting snack. Paramedics arrived.

Was fine for ~8 hours then started to feel ?off? after completing an upper body workout. I then felt nauseous and threw up, and my body began to ache. 12 hours post vaccination I began to develop a fever of 99.8 with the use of acetaminophen and had severe chills / muscle aches. The fever progresses to 101.1 about 36hours post vaccine and declined to 100.8 48 hours post vaccine. By 72 hours the fever was gone as well as all symptoms excluding fatigue and a slight headache.

A few hours after the injection, I started feeling severe joint and back pain. Around 5 PM is when I started feeling a tingling feeling in my hands, feet, and around the outside of my mouth. I still have a loss of feeling in my feet after returning to work today, 1/25/2021, and I was promptly sent home after informing my management. I still have some loss of feeling around the corners of my mouth, and most of the feeling has recovered in my hands.

Moderna covid-19 vaccine EUA. Night sweats, body aches lasting 72+ hours, body shakes, hallucinations at night, headaches, dizziness lasting 72+ hours, extreme fatigue lasting 72+ hours, sore throat, joint pain, nausea , pain at injection sight lasting 72+ hours

On 2/24/21 around 1:30 PM, Patient was not feeling well and temperature was 100.8. He was able to take Tylenol but then the staff noted his eyes rolled back in his head. 911 was called but he was not transported since his temperature was now normal and he seemed to be at baseline. He did not eat or drink much liquids later that night and in the morning of 2/25/21 around 9 AM, staff could not get him up from bed. He was incontinent and had temperature of 97.7. He was staring off and not talking or responsive. 911 was called and he was transferred to the ER and later admitted. All labs and Xrays were normal and later on 2/26/21 he was awake and alert. Of note, Patient also received news on the afternoon he got his second vaccine that a favorite staff person had been killed in a MVA. Patient was upset at this news at the time.

I did not feel anything shortly after the injection, but later in the evening, started to develop a normal post-vaccine soreness. I thought little of it and went to bed. I woke up just after 3am with chills, fever, and worsened arm pain, to the point that it restricted my arm's range of motion and speed of motion. Interestingly, the arm is not red or swollen, but is painful near the injection site, and the area around my clavicle on that side is somewhat swollen, but not red or painful (likely a lymph node). I was unable to get back to sleep due to these side effects (primarily chills and fever). After a few hours of trying to get back to sleep, I took some Tylenol and tried to eat breakfast. I was unable to keep much down, and had to arrange a day off of my internship, and was finally able to sleep on and off for the next few hours. The symptoms have somewhat lessened, but have not resolved, after about 22.5 hours post-injection. I fully expect this to resolve or mostly resolve by the 48 hour mark. I mostly found this unusual due to the severity of the reaction and after only the first dose, and wondered if my young age could have something to do with the clearly robust immune response so soon after inoculation.

I was vaccinated on the afternoon of 3/5, then woke the next day with fever, chills, myalgias and headache. These persisted all day and into the next, 3/7. I was also very fatigued and slept most of these 2 days.

Fever, headache, site of injection in deltoid sore, nausea, loss of appetite, weakness, lack of sleep, all within 24 hrs

Patient warned me prior to administering the shot that he had a phobia to needles. He did very well during the shot, but while waiting in the observation area he was speaking to another customer about needles and after she described a bad experience with an iv he went pale and slumped in his seat. The other customer brought this to my attention and I checked on patient, who had dropped all this things on the floor by the chair and he said he was having a panic attack. He said he felt very hot so we got him a bottle of water and fanned him until he started feeling better. After about 15 minutes he said he was feeling much better and said he thought he might have blacked out for a moment because he didn't remember dropping his things. He said he had rushed here after work and was hot and tired and that may have contributed to the panic attack. His mother came and drove him home.

I kept a log of the events that occurred. Notes: 25mg amitriptyline taken nightly for the last month, 200mg motrin I?ve never taken pain killers before this, time is in military. Mar 18 1430 (1 hour 30 mins after my shot) i have a compressing headache toward the back of my head so i take 1 motrin. 1700 i head home, take 1 amitriptyline and head straight to sleep. ( i have no apatite so no dinner ) Mar 19 0100 The pain starts, it?s unbearable. I am sweating heavily and im also shivering cold. i have a high fever I have body aches which turns my shivering into shaking which keeps my wife up all night. In my dreams i feel pain. I talked to my deceased mother, i hold my deceased cat and i relive some of my most regrettable moments. 0700 I wake up, the bed is soaking wet. I take a motrin and get dressed. it takes me an hour longer to get ready for work because, the pain has sapped my energy. My supervisor sends me home seeing how horrible I look. 0900 I take an amitriptyline and lay down. I ask me wife to close to the storm shutters to darken the room. They are normally pretty quiet to close but, this time they are deafeningly loud. I struggled to get 1 hour of sleep. 1500 I wake up due to shaking so bad that i hit my head on the wall. I then take 2 motrin and about 30 mins later i notice theres a tightness in my chest. i take 1 tums. I know that my body is exhausted from the pain and my mind is clouded but i really wanted to do all of the tasks that i was planning on doing this weekend. All of the tasks end up failing. 2100 i try the tasks again and end up failing again. 20 Mar 0100. I eat a pear and 2 cheetos. i do not take a amitriptyline and i lay down. i have no pain or fever. (Dream start) The dream i had is of me watching myself get thrown around by a ghost. The ghost possesses my body for a short time. I grab some kittens (cats are a comfort to me so im guessing that my subconscious trying to reel me back in) I then wake my sleeping wife and tell her theres a ghost and we need to leave. We walk outside and im in an old neighborhood that i grew up in, everything darkens and im in pitch black feeling my way around a room. I open my eyes and im in a hotel. The front desk staff will not let me take the kittens up to my room so i leave them. When i walk into the room, it?s full of kittens (dream end) and i wake up 21 Mar 0900 I wake up and I have a sore throat and tightness in my chest. The soreness prevents me from going back to sleep and the tightness in my chest causing some anxiety. Once I calm myself down and convince myself that im not having a heart attack I take 2 motrin and some tums and the pain goes away from the day. These could be some withdrawals from not taking amitriptyline since 19 mar in the morning. This is still happening as I write this report. (23 March 2021)

T+4hr: Tenderness at injection site T+12hr: Tiredness, Ibuprofen 200mg T+24hr: Dizziness, confusion, drowsiness, fever (37.5C), Ibuprofen 200mg T+36hr: Neck stiffness

immediately started to perspire after the shot. then had behavioral change. usually well behaved at baseline on his medications, but he started acting strangely and even tried to strangle his mother. she notes that his eyes were angry and looked like he wanted to kill her.

Severe rash on toes, feet, legs, mouth, hands, and upper arms. Swelling in feet and toes. Injection site has become very hot to touch. Rashes began on 04/03/2021 on just the back of heels but then spread quickly to top of feet and toes within just a few hours. Used diphenhydramine gel and cream on feet on the night of 04/03 to be able to sleep. Rash on injection site and on the same region of opposite arm intensified on 04/04/2021. Intense itchiness called for more ointment on both upper arms injection site still hot to touch and ring rash has become more visible. Foot and toe rash returned at 8 pm 04/04/2021 and is unrelenting. Bumps appearing on toes.

PATIENT FELT DIZZY AND NAUSOUS AFTER VACCINATION. PATIENT SAID HE LOST COLOR IN HIS VISION. PATIENT'S HEAD ROLLED AND THEN HIS WHOLE BODY BECAME STIFF BEFORE COMING BACK TO CONCIOUSNESS. PATIENT SAYS THIS HAPPENS WITH ALL VACCINATIONS AND DRAWING BLOOD.

Fever (up to 102F), chills, slight nausea, tension headache on upper neck, restlessness, joint pain, all beginning around 5pm. Went to sleep at 8-10pm (challenges sleeping). Woke up at 8am with 99.0F temp. Showered. Back to 97.6 by 10am.

Developed a skin rash that was localized to the back side of my arms. Since then the rash has worsened severely and spread to my neck, torso, and back. I have seemed medical care multiple times. Been given 2 different steroid creams, 3 different antibiotics. The most recent visit (2 days ago) I was given Pepcid and Zyrtec. At this point I cannot sleep due to the heat and pain coming from the sites of the rash and need something to resolve it

Became extremely exhausted by the evening with slight back pain. At about midnight, woke up with fever of 102, severe back pain, and cold sweats. Did not sleep through the night. Woke up with vomiting, severe body aches, cold sweats, shivering, continued fever of 102. Took 2 tylenol every 6 hours and fever did not drop below 100 at any time. Had severe body aches mostly in the back. Slept for a few hours at a time the day after the vaccine. Constant shivers, sweating, vomiting body aches. Took Tylenol before bed, woke up the next feeling. Second day to present some brain fog, but not other side effects continued on.

04/12/2021 03:00 pm Responded to a 24-year-old male found sitting on a chair in the 30-minute observation section with c/c anxiety/heart palpitations. Patient was a/o x 3 with GCS 15. As per patient, he had sudden onset heart palpitations and stated that he felt 'anxious.' Patient stated that he felt nervous to for the second vaccination and that he felt 'more nervous when placed in the 30-minute observation section.' Patient was taken into tent for further evaluation by HCP. Patient had history of anxiety. (-) allergies. (-) medications. (-) dizzy. (-) n/v. (-) MLAPSS. All vitals were normal. 13:15 -- 152/86 HR:75 RR:22 SpO2: 98 13:19 -- 138/82 HR:65 RR:20 SpO2: 97 13:24 -- 151/74 HR:66 RR:20 SpO2: 98 Patient was given water and food and stated that symptoms had resolved. Patient was escorted back to seat and left w/o incident. Entered medical area after being notified of patient in medical area by EMT. Upon arrival patient was sitting on cot with BP and Pulse oximeter on. No visual signs of distress. Could see patient pinching inside of upper arm and reported feeling anxious. His vital signs WNL EMT was chatting with the patient and patient was responding to all questions. Informed patient I wanted to check to see how he was feeling then excused myself outside of the area.

A 24 year old male who has completed a 15-minute observation period with the following signs and symptoms of an adverse reaction: loss of consiousness and other - dizziness. Pt endorsed he is very anxious about the vaccination. Had a hx of fainting spells during vaccinations. Pt endorsed that while he was on the chair, he felt anxious and dizzy, then tried to take deep breath and passed out. Pt was found on the ground for 5 seconds but regained consciousness right away. No headache, chest pain, SOB, swelling of lips/tongue/throat. Denies any urinary/bowel incontinence. PMHx: none Pt was sent to examination via wheelchair. Vital signs check. Given water for hydration and apple juice. Felt better. vital signs stable Have abrasion on right knee and left dorsal wrist. Physical exam unremarkable. No hematoma or laceration on the head Observed for additional 30 minutes More likely syncopal episode vasovagal This is not an allergic reaction to the vaccine

Day after receiving vaccine, patient notes progressive leg swelling, facial puffiness, and feeling of neck fullness. He estimates he has gained about 20 pounds. On 4/11, he was seen at an emergency room and diagnosed with new onset nephrotic syndrome.

Pt received Moderna Vaccine at 0915. Pt complaint of "feeling like there is a lump in my throat" at 0930. VSS and taken at 0931 (BP 125/59 sitting RA, 97% RA, 72HR, 19RR AA O x 4). Examined airway and airway appeared to be intact. No difficulty in breathing. Pt speaking in complete and clear sentences. Pt drinking water and able to maintain secretions. Pt in NAD. States he is ready to go and did not want to stay full 30 minutes for observation even though he was advised to. At 0940 - pt states he "feels fine" and left in private vehicle at that time. No further treatment was needed or requested. Pt ambulatory out of vaccination area with even and steady gait, no assistance needed.

Inability to sleep and racing thoughts throughout the night - treated with melatonin at 4:30 AM after not having slept. Slept from around 6 AM to around 11 AM Cramping joints and stiffness- untreated, stretched throughout the following day. They have mostly gone away Freezing extremities - treated with hot bath and blankets; even after treatment, freezing feeling and chills still persist Dull headache - treated with 1 200mg ibuprofen at 4:30 AM, came back after ibuprofen wore off

Patient vaccinated at 1035 - was in observation area for 30 minute observation. Patient reported to observation RN s/sx consistent with history of his anaphylactic allergy to shrimp - i.e. "throat closing" patient reported not feeling like he needed to get epi yet and did not want it. RN lead notified. Patient rapidly assessed as pale, slightly anxious but reserved. Patient was agreeable to taking 50mg Benadryl PO - given with sips of water. VS BP 140/80 P 92 Spo2 98%RA. - Stat team initiated - pt rapidly assessed and transferred to ED per patient request. Chart review indicates patient medicated in ER with Solumedrol, famotidine and loratadine and given a bolus of 1L of NS. VS remained stable throughout. Discharged with 2 days of 20mg Prednisone PO daily. Pt has supply of epi pens at home.

Pt was sitting in post-vaccination area for his 15 minute observation time when he passed out after a few minutes. He had fallen onto the floor beside his chair. I immediately went out to pt. His eyes were fluttering. I shook pt and asked if he was okay. Pt opened his eyes and sat up seemed disoriented and said that he felt nauseated. I told pt to lay down on his back and elevate his feet on the chair, and told my technician to grab a bucket, an ice pack, and to call code white. We applied the ice pack to pts head. A member of management and an EMT who happened to be in the store reported over very shortly. Pt said he did think that he hit his head and his head did hurt. We asked the pt if he wanted us to call an ambulance. Pt said no, he was feeling a little better and he just wanted to wait a little bit and then finish his shopping. Pt was feeling better and was able to get back into the chair and sit. Pt appeared pale and we got him a drink of water. Pt sat and rested for sometime and after about an hour he said he was okay and was going to finish his shopping.

Systemic: Fainting / Unresponsive-Mild, Systemic: Flushed / Sweating-Medium, Additional Details: Patient was extremely nervous and sweating prior to the shot. Patient was already sweating through his shirt and pants. After the shot patient fainted. It was likely due to the extreme nervousness and not an adverse reaction to the vaccination.

Patient said he could "really feel the shot" after he sat down, I asked him what he was feeling, and he said he felt a little bit dizzy, so we decided we would go ahead and lay down on the floor and put his feet up in the air just in case he fainted. He said he felt normal shortly after, I monitored for another 10 minutes, then tried sitting upright in a chair again for 10+ minutes, felt fine before heading out.

Pain in arm at injection site started about 3 hours after injection. Full muscle pain and painful to lift or move arm for next 3 days. Dizziness/lightheaded starting around 7PM after moderate walk, then persisted for next 3 days. Fever of 99.5, strong chills and shaking, and mild nausea started at 10PM as I prepared for bed, persisted through the night, but chills and nausea gone in morning, and fever mildly subsided. Took NyQuil before going to bed to treat fever. Chills returned next night, but much milder than before and for shorter. Took ibuprofen, 2 tablets twice a day for next two days. Dizziness and mental fog persisted until 3 days after injection.

I?ve been having palpitations, headaches, tremors, no longer can drink coffee, or any other stimulant, cannot do any sort of physical exercise except for walking a short distance. As soon as I carry anything I immediately start having palpitations. This has been happening for more than a month now.

He got the vaccine, mom received her shot and the pharmacist said they were done, and he told his mother that he was feeling a little dizzy and anxious and wanted to sit down. They were going to the car and he got in first as he was going to drive, and mother got in the car, and he acted like his head was spinning, had a blank stare and started making weird sounds like he could not breath, started foaming at the mouth and then he passed out. She called 9-1-1 and when they got there he was coming out of it. They checked his VS and said that everything was OK, and felt that he did not need to go to the hospital but it was their call. He didn't want to go either. She drove him home and he slept OK, he had a bad headache and felt extremely tired and lied down. He seemed to be OK the next day other than his arm being sore, but no other issues and he's been fine since. The pharmacy did not have them wait before leaving to make sure they were OK, the pharmacist told her they were fine and they could go, but the incident occurred within 5 minutes of the vaccine.

Tinnitus 18 hours after second dose. Loud high pitched ringing that gets louder during quiet, when trying to sleep. Sometimes too loud to sleep. Never suffered from tinnitus in my life, musician very careful about hearing protection. Haven't seen doctor yet but I will if it doesn't go away. Reading a lot of other people have dealt with this for weeks and months after vaccination.

Five minutes after receiving the vaccine, the patient slumped over in his chair, the pharmacist went out and shook him to wake him up. The patient was confused upon waking up. He became oriented approximately a minute after waking up. He was given water and sat in the chair for about another 10 minutes. His girlfriend came and picked him up and took him home. Patient stated he was feeling fine and has a history of fainting when receiving vaccines.

Initially I had experienced the standard adverse effects of drowsiness, a little difficulty sleeping, minor chills. The next day following my Dose 1 of the Moderna Vaccine, I started noticed chest pains on both sides of the chest. It wasn't from breathing and not very consistent or something I could re-create on command. It seemed to happen every hour for a few seconds and was a sudden, sharp pain. That was a concern for me so I did some basic internet searches and found that Chest pains are a symptom of the vaccine. I didn't think too much of it after that but It was a concern I kept in my head. My timeline gets a little foggy here but either that same day or the following day, the chest pains were still happening and I wasn't feeling 100%. One evening I went to take a bath. As I was laying in the water, my heart rate felt off, almost as if it was skipping a beat or not in the right rhythm. I remember laying in the bathtub, trying to count my pulse or seeing if I could "catch" the incorrect beat. Rather than one normal heartbeat it felt like there was 2 beats very closely timed with each other. My heart rate felt faster than normal considering I was only laying down in water and didn't do any exercises the past few days. Unfortunately I never was able to find a good way to track the pulse and timing, but I was sure that something was wrong. I had difficulty sleeping that night because I kept feeling something wrong with my heart rhythm. I got worried that I would need to go to the emergency room, but I told myself to sleep it off and if the chest pains and awkward heart beat continues for 5 days, I would go see a doctor. As for an outcome, thankfully I stopped noticing it around day 4 and haven't had chest pains since. I am extremely worried as to how my body will react to Dose #2.

PATIENT WASN'T FEELING WELL AFTER VACCINE (WAS A LITTLE NERVOUS) AND ASKED FOR SOME WATER. PATIENT SAT DOWN IN WAITING AREA WHILE I WENT TO GET HIM A DRINK. WHEN I CAME BACK PATIENT WAS RIGID AND LOCK JAWED AND NOT BREATHING. WHEN WE GOT PATIENT TO FLOOR HE WAS RESPONSIVE TO A STERNUM RUB AND HAD A PULSE. AT THIS TIME HE STARTED TO PRODUCE SNORE LIKE NOISES, CONVULSED FOR LESS THAN A MINUTE THEN CAME BACK TO. EMT'S CAME AND CLEARED PATIENT. PATIENT STATED HE HAS A VASOVAGAL REFLEX HOWEVER HAS NOT HAD AN INCIDENT IN MANY YEARS. IT IS UNKNOWN IF THIS IS WHAT PATIENT EXPERIENCED OR A REACTION TO THE VACCINE.

Pt received 2nd dose of moderna vaccine and within 1 minute started presenting symptoms of anxiety, sweat spells, and nausea. Pt was provided with water and a pharmacy bag in case of vomiting. Vomiting did not occur, only slight spit up into the bag. Pt remained conscious the entire time, no syncope occurred. Pt was observed for a time period of 30 minutes; pt felt much better after that time and was released from the pharmacy area.

Patient was not vaccinated at our site, so I do not have information regarding lot number, etc. Vaccinated with dose #2 of Moderna vaccine 10 days prior to admission. Patient began feeling poorly, with fatigue and weakness the day following his vaccination. 10 days post vaccination, mother found him with right arm weakness, facial droop, and tongue deviation. Patient was severely encephalopathic with moaning, eyes deviated, unable to follow commands, diaphoretic. No focal deficits noted. CT head negative for acute finding. Platelet count low at 24,000. LDH elevated at 2856. UDS negative. Patient suspected to have aTTP and treated appropriately with high dose steroids, plasmapheresis, and rituximab.

Numb in hand, pain in arm that radiated to jaw and across the chest. Having trouble speaking, balance issues, extremely confused most of the time. Symptoms of heart attack or stroke.

Beginning roughly May 15th, I began tapering off of diazepam to stop taking the medication at 0.5 mg a week (from original 5 mg dose); beginning around the week of July 5th, I started experiencing significant fatigue, balance issues, and memory problems. This has not resolved, but has not gotten worse. I strongly suspect this is due to withdrawal, but got the CDC phone call and decided it was worth reporting anyway, in case the vaccine increased the severity of my withdrawal symptoms.

Systemic: Confusion-Mild, Systemic: Flushed/Sweating-Mild, Systemic: Weakness-Mild. Additional Details: BP 110/62 Pulse Ox 90. Placed in rescue position and ambulance called. Patient is autistic and mother was with him. She stated he has anxiety.

Extreame left arm pain. Tender to touch and movement. Unable to sleep at night due to discomfort and pain. Unable to lift arm above head due to pain level. Pain has not decreased at all.

Pt states "I woke up, ate breakfast and around 8:30am I started feeling dizzy and having blurred vision. I was working at the time and went to my local EMS station to get checked out. They checked my blood pressure and blood sugar. Blood pressure was normal but my blood sugar was low, around 68. They gave me a coke to drink and I went back to the Sheriff's office and about an hour later my blood sugar was around 90. I took the rest of the day off and come home. I took Extra Strength Tylenol and rested. I did have a headache a few hours later. I feel fine now but I have a mild headache."

Hot flashes, chills, cold like symptoms, some elevated heart rate, injection site warm to the touch, red ring around injection sight the size of two quarters length wise, was feeling foggy headed.

After receipt of the vaccine, I developed a low grade fever the next day (1/7/21) that persisted throughout the day until it progressed into a fever of at least 101.3 F at night. It might have been higher, but that was the best reading I could get. It felt like the fever came on extremely fast bc all of a sudden I started shaking vigorously with rigors and chills. I couldn't get out of bed without experiencing the most uncomfortable chills I've ever experienced. I previously had Covid in early November, and this fever felt worse than what I experienced when I actually had Covid. Also, I experienced moderate to severe pain in my left arm on 1/7/21 at the injection site, which was far worse than any other muscle soreness I've experienced from any other vaccine I've received. The hour preceding the sudden fever attack, my left arm was feeling blazing hot, looked a little red, and swollen. I also experienced a strange allodynia type pain, whereby parts of my body hurt from simply touching them. Although I did not develop a significant rash, I experienced pruritus all over my body, predominantly affecting the areas around my ankles and wrists. I have a hx of multiple dermal filler injections in my lips. I don't think I had any problems with my lips after the injection. But other symptoms that I developed included a tickle in my throat and some shortness of air. On 1/8/21, I woke up drenched in sweat so I presumed that I broke my fever while sleeping. I have felt much better ever since. I'm a nurse practitioner student, and I had to miss a day of clinical on 1/8/21 bc of this reaction to the vaccine. I used ibuprofen the morning on 1/7/21 and Tylenol at night on 1/7/21 to bring down my fever

During the first night after receiving vaccine I started to have SEVERE shaking / full body muscle spasms-hypertonicity that lasted 60-90 minutes. Required 30 minutes hot shower to calm down shaking symptoms. Initially thought episodes were caused/triggered by cold temperature (still possibility) but now thinking its triggered by anxiety. Heart rate almost spikes to 120-130 BPM. No fever and normal blood pressure is noted when episodes happen. Occurrences is not routine and seem to be random. Very concerning considering symptoms lasted for 2 + weeks now. I do not have health care or ability to see PCP/GP without paying out of pocket. Now please understanding the severity of these episodes, to the point where it resembles severe Parkinson's symptoms. Along with those I still have the normal side effects, fatigue (extreme) , mood changes, joint aches, intolerance to cold off and on, and elevated HR.

I got the vaccine around 12:45 on a Monday - 1/4/21 4pm that day, started having a massive headache, laid in bed. Laid in bed all day, fell asleep around 10PM. 3:30AM woke up panicking with fever, tremors, chills, myalgia and very lethargic. Got out of bed to get tylenol and was shaking. Tuesday morning - woke up feeling the same as I did at 330 AM, plus no appetite on top of headache, chills, myalgia, lethargic and slept pretty much all day, felt fatigued. Wednesday was doing better, thought it was the end. Then suddenly about 7-8pm started having chills and shivering. I would say Thursday night is when I actually felt better and by Friday morning I was good with just residual ?brain fog/feeling dazed?

The day following my first COVID vaccination, I had pain in my upper arm for 36 hours and could not lift it above my shoulder. I also experienced 6 hours of constant chills throughout my body, which came on suddenly around 1PM (24 hours after my vaccination) and was sent home early from work. During the 6 hours at home I believe I experienced body chills, I experienced what I believe to be brain fog. My coworkers described my motor skills (walking down a hallway) as slow and lethargic, and my thoughts being delayed. I do not consciously remember the 6 hour timeframe when I experienced these symptoms. Around 7PM that evening, I remember having a "coming to" moment where I remember the chills subsiding, and being able to think again. The following day I felt back to normal other than pain at the injection site.

FATIGUE FOR 3 DAYS POST VACCINATION MENTAL FOG FOR 3 DAYS POST VACCINATION HEADACHE ON 3RD DAY POST VACCINATION

Patient received his 2nd dose of the Moderna vaccine prior to hospital admission at unknown site. Patient reported generalized body aches and that his whole body hurts and feels likes he's "being poked with a knife." He endorsed associated symptoms of fever, nausea, back pain, and shortness of breath. The patient denies any sore throat, headache, cough, or vomiting. His back pain is at the tip of his shoulders, in the middle of his spine, and near his kidneys. The patient adds the abdominal pain is worst in the lower left quadrant. Patient admitted for bilateral lower extremity weakness limiting him to ambulate and to rule out Guillain-Barré. During admission, had fevers, photophobia, phonophobia, and nuchal rigidity. On physical examination patient was able to minimally wiggle toes, had no plantar/dorsiflexion, unable to move hip joints or knee joints bilaterally, lift his legs against gravity.

101C Fever (2-3 days), Severe Body aches (concentrated around lower spine; 5 days), Headache (6 days), Fatigue (3-4 days), trouble talking (hazy mental state; 5 days), Lymphadenopathy (Right-side neck; 15+ days), soreness (right arm; 15+ days)

Fever, chills, headache, muscle aches throughout body, sore at vaccination site with a short lived rash, fatigue. It was bad enough to halt daily activities (I canceled a phd interview because of how bad I felt). This occurred from late 2/4 through morning of 2/6. Taking tylenol seemed to help and I felt immensely better the two or three times my fever broke.

Anxiety/panic attack after- issues catching breath, hyperventilating, fast HR rate but WNL- Forgot his inhaler at home. Had a family member pick him up and take him home for the day- advised him if symptoms persist, immediately to the ER

On March 8, 2021 @ 1438 1st dose of Moderna vaccine was given to patient in right deltoid. At 1446 patient beckoned for nurse due to feeling nauseous and lightheaded. He was lowered to the floor and assessed. Alert and oriented x3. Denies pain or discomfort. Patient is sweaty and skin pale. States he did not each anything today. Denies having medical problems. Reports having social anxiety and panic attacks. Reports allergies to cashews and nickel (metal). Denies need for EPI pen in the past. Vitals obtained and WNL. Patient offered water and given protein granola bar to eat. Stayed with patient until symptoms subsided. VS reassessed and continues to be WNL. Patient voices no other needs or concerns. Patient left vaccine site @ 1507 ambulatory, alert, oriented x 3, and without medical concerns. 1446 VS: O2: 98% RA, R: 18, BP: 132/78, P: 67 1505 VS: O2: 100% RA, R: 18, BP: 122/70, P: 76

after receipt of TWO doses of above named vaccine in the SAME day- 3hrs following vaccination, pt began experiencing anxiety, vomiting, chills and hypertension

First day night with inflammation in my deep nose and feeling chills. Second day the mucus start to come out and had sore throat. At night, I had the fever and the headache that I couldn't even sleep. I took the coughing syrup and barely slept.

Systemic: Fainting / Unresponsive-Mild, Systemic: Seizure-Mild, Systemic: Shakiness-Mild, Additional Details: pt came in for his first covid shot. checked pt health status pt was fine. while pt was at the observation seat his head went back and he started to have minor sshakiness/seizure like/faintness for about 5 sec. paramedic came and pt refused the pramedic he was checkout by paramedic and was responsive. pt then told us he had been fasting for 2 days and had marijuana on his injection day. pt went home and called his primary dr. and is doing well.

Patient describes dizzy from panic attack/anxiety and lowered himself to the floor. Patient said he would be okay, it has happened twice before (before Dr. appointment). We helped him to a chair right next to him, I took his blood pressure (around 99/70). It was normal range, but low for the patient, he reported. We waited about 15 minutes and he said he was feeling okay and did not want EMS. We had the manager of the store take him home.

Minutes after vaccination patient reported feelings of lethargy, light headedness, and unable to focus. He instructed patient to sit and relax for 30 minutes. Prior to completion of 30 minutes, patient felt he might lose consciousness and we administered an ammonia smelling packet. Patient was more alert but still feeling symptoms. We had patient eat crackers and orange juice. After 15 additional minutes patient reported feeling normal and okay to leave.

Approximately 10 minutes post vaccination, the patient expressed feeling "funny" and that his throat felt tight and he felt he was about to start sweating. His hands appeared red and were warm to touch, no other flushing noted. Blood pressure and pulse elevated. Benadryl 50mg administered PO. After 45 minutes of observation, vital signs returned to normal, patient noted resolution of tight throat feeling, feeling "funny" and redness of hands. D/C'd home with education regarding epipen use and emergency reasons to contact 911.

First dose: some arm soreness and little pain during this time Second dose: did not start off that bad, felt pain when moving arm around (lifting arm) around 3-4pm - felt feverish (about 12 hours after dose), couldn't sleep, felt chills and that lasted until next day - few hours after injection until the next day, felt generalized exhaustion, muscle aches and fatigue - few hours after injection felt nauseous and it went away after waking up from night sleep the next day - took ibuprofen 800mg around the time of injection and then took another ibuprofen 800mg in the evening

I had an anxiety attack. The physical symptoms were a severe headache, fever, vomiting, shaking, and aching body.

Patient complained of feeling anxious a few minutes after receiving the vaccine, while he was just about to walk up stairs to the observation area. . Stated, "I could not see, my eyes went black, and my fingers were tingling. I almost passed out". Clinician immediately sat patient on a chair and gave him water, which he drank. Medics were called to see patient. Vital signs were WNL. Once cleared by medics, patient was escorted to the ground level observation area. He soon reported that he was back to baseline. Medics cleared him as being medically stable. Patient reported that he no longer was light headed and had no visual disturbances. {Patient did say that he often gets anxious with needle injections.

Patient became very pale, shaky, and said he felt as if he were going to faint immediately after receiving vaccination We had patient lie down, checked his blood pressure which was a little low We gave patient some glucose tablets, a coke to drink and placed cool damp towel on his head Patient stated he had not eaten anything since the day before and was very nervous about getting shot/afraid of needles when he got to pharmacy Patient began to get his color back and felt well enough to sit up in a chair We then gave him some peanut butter crackers to eat Patient stayed in pharmacy area for 30 - 40 minutes until he felt better and wanted to leave We offered to call an ambulance for him if he felt like he needed to seek medical attention but he stated he did not

Systemic: Allergic: Anaphylaxis-Medium, Systemic: Confusion-Medium, Systemic: Dizziness / Lightheadness-Medium, Systemic: Fainting / Unresponsive-Medium, Systemic: Flushed / Sweating-Medium, Systemic: Hyperventilation-Medium, Systemic: Hypotension-Medium, Systemic: Shakiness-Medium, Systemic: Weakness-Medium, Additional Details: immediately after first dose pt head started to lower, eyes closed, slid down in chair. respiratory rate decreased for several seconds then panic breathing occured thereafter. pt was sweating profusely and skin was cold and clammy. could not verbally respond for 30-45 seconds. after a minute pt could talk and answer question. blood pressure was taken 3 different times 5 minutes apart 75/43, 87/46, 93/52. Pt was feeling fine a half hour later and was called 4 hours later to follow up.

Severe chills and high fever (102). Severe muscle aches and insomnia for 1 day. Symptoms lessened after 24 hours. Fully recovered after 48 hours.

Patient is a 25-year-old male with past medical history of depression with anxiety, esophagitis and hemorrhoids. Patient presented with petechial rashes all throughout his body started on Friday 4/9 after getting a tattoo the day prior and his first moderna covid vaccine 3/28. On admission patient was found to have a platelet count of 1. Heme/ onc was consulted and thrombocytopenia was deemed secondary to ITP. Hepatitis B panel, showed previous hepatitis B infection, although results may be inaccurate due to patient receiving IVIG prior to hepatitis panel being done. Patient also had an equivocal syphilis result, which also could be a false positive due to immune response. Patient received 2 IVIG, 4 days of Decadron, and 1 dose of Nplate. Patient did not receive rituximab, due to stable increasing platelet count. Patient received hepatitis-B prophylaxis for 2 days in case he were to receive rituximab. Platelet count now stable at 25, to follow-up with hematology in 1 week outpatient for repeat CBC. Patient also to follow-up with his Primary Care Physician in 1 week.

Vaccine administered 8:32am. At 8:36am VS BP 74/51, P 40, R18 T98. pt sitting on chair and fell on right lateral side. Pt noted with bump to right forehead. Pt A&O verbalizes feeling dizzy and nervousness. Pt diaphoretic with brief episode of seizure-like eye twitching. Pt denies any chest pain. 8:39am VS BP 90/40 P50, R18, T98 O2 Sat 98%. Lead nurse made aware. Md on call made aware. As per MD notify EMS. Orders carried out. Applied cold compress to forehead while pt sitting up. Wife present. VS 8:42am BP 128/76, P74, R18, T96.8, O2 98%. Pt under no distress. 8:52 EMS arrived to site. Pt evaluated by EMS and offered to be taken to hospital for further evaluation. Pt refused. Advised pt of consequences for leaving AMA. Pt verbalizes understands. 9:01am pt signed AMA. Instructed pt to seek further medical assistance if signs/symptoms persist at home. Assited pt to exit site on wheelchair accompanied by wife.

Patient was given vaccination, proceed to wait 15 minutes after given vaccination. Was seated in chair in waiting area, fell out of chair due to Vasovagal syncope. When patient came too he was disoriented but breathing on own and could answer simple questions of name, place etc. Checked pulse and %SpO2 both within normal. Patient was allowed to lye on floor for 5 minutes or so then propped up against wall for another 5 minutes. After observing for about 10 minutes pharmacist decided to call paramedics (just as a double check ) to make sure all ok. Paramedics came and checked BP/Pulse/%SpO2 all within normal. About 15 minutes go by and pharmacist drives patient home, and patient picks up car later. On drive home everything nothing observed abnormal.

Patient came for second dose of Moderna vaccine and upon reviewing PQCF the patient indicated that for his first dose during the 30 mins observation period he experienced difficulty swallowing and dizziness. The patient has a history of anaphylaxis with sulfa drugs. Patient did not inform of adverse event to his PCP nor the staff of the pharmacy. He was very nervous. He was advised to consult with his PCP the risk vs benefit of receiving his second dose and to inform us of the final decision for documentation. Second dose of vaccine was not administered until further communication between patient and PCP.

First night after receiving 2nd dose of the Moderna Vaccine, I experienced a high fever and chills, sweating and a disturbing Ringing in my head. The second day after was roughly the same as the first night. I expected some of these symptoms and was willing to power through them. It is now past 2 weeks since I have had the Vaccine. I still have a maddening Ringing in my head. Different than a Ringing in my ears. It does not feel the same. I feel pressure in my head, resulting in constant headaches that never subside. I have felt nauseous at random times, stomach pain from not being able to eat, and I am for the first time in my life experiencing anxiety and depression. I feel like my pain will never go away and I am scared. This pressure/pain in my head accompanied with the constant ringing is driving me insane. I have lost time at work, lost time with friends and family trying to sleep off my pain and symptoms but it never helps. I have taken headache medication which helps a small amount for the head pain, but leaves me with an upset stomach. I can not take these pain pills any more. My Doctor, had me do a blood test and we are awaiting results as of today 5/25/21. I do not think they will find anything that will be able to help me with what I feel this Vaccine has done to me. I do not know what to do besides try not to cry myself to sleep anymore. I hate this and I do not want to feel this way anymore.

motion sickness in car; micro movement of the eye; lack of focus of the eye; prolonged dizziness; intense headache; Dropping blood pressure; Myalgia; This spontaneous case was reported by a consumer (subsequently medically confirmed) and describes the occurrence of MOTION SICKNESS (motion sickness in car), EYE MOVEMENT DISORDER (micro movement of the eye), OCULAR DISCOMFORT (lack of focus of the eye), DIZZINESS (prolonged dizziness), HEADACHE (intense headache) and HYPOTENSION (Dropping blood pressure) in a 25-year-old male patient who received mRNA-1273 (Moderna COVID-19 Vaccine) (batch no. 040B21A) for COVID-19 vaccination. The occurrence of additional non-serious events is detailed below. No Medical History information was reported. On 12-Apr-2021, the patient received first dose of mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) 1 dosage form. On 12-Apr-2021, the patient experienced MOTION SICKNESS (motion sickness in car) (seriousness criterion hospitalization), EYE MOVEMENT DISORDER (micro movement of the eye) (seriousness criterion hospitalization), OCULAR DISCOMFORT (lack of focus of the eye) (seriousness criterion hospitalization), DIZZINESS (prolonged dizziness) (seriousness criterion hospitalization), HEADACHE (intense headache) (seriousness criterion hospitalization) and HYPOTENSION (Dropping blood pressure) (seriousness criterion hospitalization). On an unknown date, the patient experienced MYALGIA (Myalgia). At the time of the report, MOTION SICKNESS (motion sickness in car), EYE MOVEMENT DISORDER (micro movement of the eye), OCULAR DISCOMFORT (lack of focus of the eye), DIZZINESS (prolonged dizziness), HEADACHE (intense headache), HYPOTENSION (Dropping blood pressure) and MYALGIA (Myalgia) outcome was unknown. DIAGNOSTIC RESULTS (normal ranges are provided in parenthesis if available): On an unknown date, Blood pressure measurement: mmHg (Low) Dropping blood pressure. On an unknown date, Cardiac stress test: (Inconclusive) Inconclusive. On an unknown date, Magnetic resonance imaging: (normal) had a cyst but nothing that would cause symptoms. The action taken with mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) was unknown. Treatment medication includes antibiotic for cyst, 25mg 3xday meclizine for inner ear, and Midodrine HCL 10mg for dropping blood pressure and 10mg Pregnisone for inner ear inflammation. Patient underwent VNG test had no inner ear inflammation and noticed micro movements of the eye No concomitant medication information were reported Caller wanted to report adverse event. Callers first dose was on 12apr21, LOT#040B21A, Expiration date 10/2/21. Caller said his life has been upside down since he got the shot. First week had intense headache and dizziness. Second week he had muscle aches and dizziness with limited headaches. Third week he is having a focusing and dizziness issue. Caller said he seen doctor and specialist. Caller had MRI and Stress test. Caller said MRI came back clean. Caller said they had a cyst but nothing that would cause symptoms. Caller said Brain scan came back negative. Caller said he was completely heathy. Caller said he also gets motion sickness now since he had the vaccine. Primary care doctor gave antibiotic for cyst. PCP thought had something to do with inner ear, 25mg 3xday meclazine, then the gave Midodrine HCL 10mg for dropping blood pressure. Caller said he got relief for 3 days then he started getting sick. Caller went to ER and they took him off the medicine. Caller when to a ENT doctor. Caller said took VNG test. He was prescribed 10mg Pregnisone for inner ear inflammation. The VNG test proved he didnt have inner ear inflammation. Caller said his symptoms have been going on for 32 days as of yet. CAller said he cant go to restaurants because of dizziness and sickness. Caller said during his VNG test they noticed micro movements of the eye. His ENT thinks this may be causing the dizziness and lack of focus. Caller said he will travel to anywhere to get help.; Sender's Comments: Based on the current available information and temporal association between the use of the product and the start date of the events, a causal relationship cannot be excluded. Further information regarding detailed medical history and diagnostics will help in assessment.

1: He had his second Moderna vaccine shot at noon May 12,2021. Within one hour, he consistently had hiccups, then followed vomiting. It last for 24 hours without any food and sleeping. 2: 5/13/2021, he was sent to Urgent care, had infusion, and Ondansetron. But he constantly had hiccups and vomiting. He could not fall asleep due to the illness.3: 5/17/2021, his vomited dark brown color material, and his defection had black color. He was sent to urgent care second time, but transferred to ER at hospital.4: 5/24/21, he started hiccups and vomiting again. His 5/25 blood tests had significant problems in liver, kidney and pancreas ( details in blood test reports). These test results make us very anxious.

Patient c/o chest pain, shortness of breath, headaches, sweaty and shaky. EMS was called and patient refused to go on ambulance and left building with transportation.

Patient compliaing of lightheadedness and headache after receiving the 2nd COVID vaccine Moderna at 13:12. Two EMT's reported to Hospital on site of similar reaction to 1st dose of Moderna. At 13:32 Client complains of lightheadedness and headache, after 20 minutes in observation. Clients vitals blood pressure: 144/86, heart rate: 79, respirations: 14. oxygen saturation 97%. Client mentions history of lightheadedness nausea and headache during his first Moderna vaccine. Client is alert and oriented to person, place, time and date. Client denies nausea, chest pain, abdominal pain, shortness of breath, or any noticeable rashes or itchiness. At 13:38 Client states headache worsens rating at a 6/10 pain scale, and lightheadedness, Client states "head feels fuzzy" and had breakfast at 1100am. Clients vitals blood pressure: blood pressure: 131/86, heart rate: 70, respirations: 14. oxygen saturation 98%. Client was given water and juice and had client move to anti-gravity chair. At 13:44 Client states headache continues to be a 6/10 and "comes in and out", clients vitals blood pressure: blood pressure: 142/87, heart rate: 64, respirations: 14. oxygen saturation 98%. At 13:54 Client stated headache worsens to 7/10 on pain scale and still feels lightheadedness. Client stated that he has a driver that will take him home. Clients vitals blood pressure: blood pressure: 131/80, heart rate: 66, respirations: 14. oxygen saturation 97%. PHN on site advised client to contact emergency medical services or follow up with primary care provider if conditions worsen. Client agreed and verbalized understanding about information provided. At 1402, clients vitals blood pressure: blood pressure: 127/85, heart rate: 66, respirations: 14. oxygen saturation 97%. Client was escorted to parking lot with EMT at 14:08 with a steady gait.

Patient was walking in waiting room, passed out, fell backwards and hit his head on a chair. When I came to the waiting room patient was actively seizing and bleeding from a laceration on the posterior part of his scalp. Patient moved from under chair and placed in recovery position. Active seizure time from my arrival was 30 seconds. Patient was minimally responsive and postictal for approximately 5 minutes and diaphoretic. Vital signs taken and oxygen placed on patient. Color returned to normal, and VSS. BS was 108 on glucometer. EMS called and patient transported out of facility. Patient is a 35 yo M with no sig pmhx presenting after syncopal event after receiving his COVID vaccine. Fell from standing, denies dizziness, SOB, CP, or other symptoms prior to the event. He hit the back of his head when he fell. He presented to the ED, at which time he felt very nauseous and per notes he was confused and perseverating. Admitted to the SICU for R frontal IPH and R frontal SDH.

Dizziness, nausea, chills lasting 20-30 minutes tightness in chest / light pain near heart, lump/swelling in throat lasting 5-6 hours then tapering down for the next 4 hours

After 2-3 minutes given the shot, patient reports feeling ill, nauseous, dizzy and sweaty all over. He denies difficulty of breathing, hyperventilation, or rashes. He asked to use the bathroom facility in case if he vomit. He did not vomit. He splashed water onto face and reported that made him felt better. He rested on a sofa in the pharmacy and was given a cup of water. After 3-5 min, he reports all symptoms have subsided and thought maybe he was too anxious due to anticipation of severe reactions after the second shot. Patient continues to deny trouble of breathing nor rashes throughout the 20 minutes resting in the wait area. Benadryl & Epi-Pen were not needed nor given.

Fatigue and headache, then after 2 days followed by severe sharp chest pain like being stabbed in the chest and could not sleep

Pt was administered Moderna on 07/14/2021. Pt called on 7/20/2021, left a Voice Mail stating he would like a call back to discuss side effects to the vaccine. I called patient back the morning of 07/21/2021, the patient states to me that two hours after the vaccine was administered he started to shake uncontrollably and was very cold, then he blacked out (Patient states he may have had a seizure.) Pt woke up outside behind his vehicle in a pool of his own vomit. I asked if 911 was called and the patient said no because he could not move or walk. The patient also stated that he felt bad all day, Thursday (07/15/2021), the day after the vaccine was administered, woke up on Friday (07/16/2021) feeling relatively normal still feels the urge to vomit when eating certain foods.
