Client came to be vaccinated accompanied by her father on Saturday January 23, 2021. The greeting table in the front was very busy and a lot going on. The client had registered online and had her paper to prove it when she was escorted to he next tables to be registered. There was nothing adverse about her registration form. Date of birth was listed, however, it was not caught that she was less than 18 to receive Moderna. We advertise that it is the Moderna vaccine that we have. Father never said anything and stood off to the side waiting for her to be observed for 15 minutes. On Monday, January 25, I spoke with client who answered phone, to explain what happened and to see if she was OK. States her arm at injection site hurt a little, but nothing else. I said I would call each day to make sure she was OK. Later in the day, she left a message to call her. I called and she wanted to know what vaccine she would get as her second dose. I said it would not be Moderna and that I could find some information and call her back. Then she states she has a lawyer and to whom should she address it to. I said the Health Officer at the city.

16 year old inadvertently received the vaccine; sore arm; supposed to receive the Pfizer vaccine but received the Moderna covid-19 vaccine by mistake; A spontaneous report was received from a healthcare professional (HCP) concerning a 16-year-old, female consumer who received Moderna's COVID-19 vaccine (mRNA-1273) was less than 18 years old and had a sore arm. The patient's medical history was not provided. No relevant concomitant medications were reported. On 06 Jan 2021 prior to the onset of the events, the patient received their first of two planned doses of mRNA-1273 (Lot number : 025J20A) intramuscularly in the left deltoid for prophylaxis of COVID-19 infection. On 06 Jan 2021, a 16-year-old patient was inadvertently vaccinated. The patient was supposed to receive Pfizer vaccine but received Moderna COVID- 19 vaccine by mistake. The patient experienced a sore arm but no other known symptoms. No treatment was received for the sore arm. Action taken with the second dose of mRNA-1273 in response to the event was not reported. The event, vaccine given to a patient less than 18-years-old, and wrong drug administered was considered resolved. The outcome for the patient's sore arm was not reported.; Reporter's Comments: This case concerns a 16-year-old female patient who received their first of two planned doses of mRNA-1273 (Lot unknown), reporting Product administered to patient of inappropriate age and Wrong product administered with an associated non-serious expected event of Injection site pain. Had very mild pain in left arm; 16 year old daughter injected with Moderna vaccine; A spontaneous report was received from a consumer, concerning a 16-year-old female patient, who received Moderna's COVID-19 vaccine, where 16-year-old daughter injected with Moderna vaccine had very mild pain in left arm. The patient's medical history reported included severe asthma. Concomitant product use was not provided by the reporter. On 28 JAN 2021, prior to the onset of event, the patient received their first of two planned doses of mRNA-1273 (lot number unknown) intramuscularly in the left arm for prophylaxis of COVID-19 infection. On 28 JAN 2021, a 16-year-old patient was injected with vaccine and had no reactions of fever or any other adverse events. Later, on an unspecified date in 2021, she also experienced very mild pain in her left arm where she was vaccinated. The patient was under asthma medications twice a day for her asthma. Action taken with mRNA-1273 in response to the events were not reported. The outcome of the event, 16-year-old daughter injected with Moderna vaccine was considered resolved on 28 JAN 2021. The outcome of the event had very mild pain in left arm was considered unknown at the time of this report.; Reporter's Comments: The events developed on same day after first dose of mRNA-1372. Received the COVID vaccine at age 16; Injecstion site pain; A spontaneous report was received from a physician regarding his daughter who is a 17-year-old female, unknown race and ethnicity, who was administered Moderna Covid-19 vaccine and experienced pain at injection site. The patient's medical history was not provided. No concomitant medications were reported. No information on allergies. On 22-Jan-2021, prior to the onset of events, patient received her first of two planned doses of Moderna Covid-19 vaccine intramuscularly for the prophylaxis of Covid-19 infection. (Lot#011J20A). The physician who is patient's father would like to give her the second dose. Also, he wanted to consider her for pediatric trials. Action taken with vaccine is physician showed willingness to give his daughter a second dose. The outcome of the events, inappropriate age at vaccination and pain at injection site is unknown at the time of report.; Reporter's Comments: This report refers to a case of product administered to patient of inappropriate age (17 year old) for mRNA-1273 (Lot # 011J20A) with pain at injection site reported. Injection site pain and tenderness; 16 year old patient administered the vaccine; A spontaneous report was received from a health care provider regarding a 16-year-old female patient who received Moderna's COVID-19 vaccine (mRNA-1273) inappropriately at the age of 16 and experienced injection site pain and tenderness/vaccination site pain. The patient's medical history was not included. Products known to have been used by the patient were Vitamin D3 and ethinylestradiol, ferrous fumarate, norethisterone. On 12 Feb 2021, prior to the onset of the events, the patient inappropriately received the first of two doses of mRNA-1273 (Lot number: 031M20A) intramuscularly in the right deltoid for prophylaxis of COVID-19 infection. On 13 Feb 2021, the day after the vaccine was given the patient experienced injection site pain and tenderness. Treatment information was not provided. Action taken with mRNA-1273 was not reported. The outcome of the events, vaccine being given inappropriately to a 16-year-old, was considered resolved on 12 Feb 2021. As patient was in the upstairs lobby the check-in nurse caught that the patient received Moderna as their first dose on 03/21/2021. Patient is 16 years old and was returning today for their second dose. Check-in nurse alerted myself Nurse Co-Lead. Nurse Co-Lead elevated the situation to Clinical Operations Coordinator, whom verified and approved for patient to receive the second dose of Moderna according to CDC interim considerations. Nurse Co-Lead verified that patient received the first dose of Moderna at the health center and on the patient's immunization record. First 0.5 ML IM dose of Moderna lot #046A21A administered on 03/21/2021. Second 0.5 ML IM dose of Moderna lot #042B21A administered on 04/18/2021. Patient denied adverse reaction to both doses of Moderna, only slight soreness at vaccination site for first dose on 03/21/2021. Nurse Co-Lead then spoke to all Moderna vaccinators and ancillary staff assigned for 04/18/2021, I reiterated the correct age groups for both Moderna and Pfizer. Our vaccination sites have today's date for allowing to vaccinate to which age group and for which vaccine on the walls of each nursing station within the vaccinator's view. In addition, our reception desk inside the building and now outside check in table have these daily reminders visibly in front of them. Nurse Co-Lead spoke to the nurse specifically that administered the first dose of Moderna on 03/21/2021 and re-educated the medication rights and agreed to the methods in preventing this error in the future. Nurse Co-Lead spoke to Lead Ancillary to re-educate and reiterate her ancillary staff each day as well. Nurse Lead produced a new nurse station safety checklist that includes medication rights, correct age group for each vaccine, and verbal consent from patient, and the 3 contraindication questions for the vaccinator/nurse to hold the vaccine. Nurse Co-Lead spoke to lead registration table ancillary lead as well as the site manager that oversees the ancillary staff outside the building as initial check in staff. Nurse Co-Lead and Nurse Lead agreed to have end of day huddle to re-iterate today's captured vaccination error and methods to prevent in the future. Patient along with her legal Guardian verbally consented and agreed to complete Moderna series.

Starting 5 days after her 1st Moderna COVID vaccine, she started to break out in a pruritic migratory rash, abdomen, trunk, arms, feet, face, comes and goes, migratory, looks consistent with known atopic dermatitis, worsening after the vaccine with nothing else new introduced. She also had body aches and viral symptoms 5 days post-vaccine. The rash is still present 11 days post vaccine.

17 year old female , - received vaccination at event. Her parents approved of vaccination - she did not disclose age prior to vaccination . Would like to proceed with 2nd vaccination - parents approve . Minor consent to be signed Minimal arm soreness post injection

Patient arrived to clinic for COVID-19 Vaccine (Moderna), passed pre vaccination checklist for COVID-19 Vaccines. Oversaw age, due to patient passing screening and fitting into criteria for vaccinating phase. No adverse side effects have been stated up to current date. reported sore arm (area of injection), and chills, which are expected after COVID-19 vaccine administration.

Patient given 1st dose of Moderna at age 17. Scheduling system did not flag her DOB as ineligible as it normally does. Patient's only symptoms were mild to moderate fatigue lasting one day and mild to moderate injection site soreness. No other symptoms reported. Patient's mother reports that patient is "still doing very well" as of today, 3/30/2021. Public Health will notify patient's primary care provider.

Reported that 17yr old client received the Moderna vaccine against current authorized use based on age regulations; s/w parent/ mom who reports post vaccination, client experienced elevated temperature (103.6) and localized site soreness; denies any present symptoms or additional complaints at this time.

Vaccine was given to patient that was under 18 years of age. Patient had soreness in left arm on day of injection. Troubleshooting: The vaccinator checked ID but did not catch age < 18.

No unusual reaction at time of injection; expected soreness on the following day. One week later, injection site and surrounding area became inflamed, hot, swollen, and excessively itchy. Reaction lasted approximately 24 hours before fading.

Pt received her 1st injection of the series at the age of 17 versus the recommended age of 18 y/o. Nurse called and spoke with mom to notify of the finding and to check in on pt. Mom reported that pt experienced soreness at the site with mild swelling and a low grade temperature. Mom reported that pt denied the suggestion of any medication and s/s resolved in less than 48 hours. Mom reported that pt would be returning to health center as scheduled for second dose per CDC recommendation.

swelling and redness to injection site. Hives to neck. Pt reports of sore throat limited to the side of the injection.

Itching, brief shortness of breath. No wheezing. No hives or rash. Shortness of breath immediately subsided. Itching subsided after 40 min.

Patient was screened and identified allergy to PCN. Covid Vaccine administered without incident. Nurse did insist client stay in area for 30 minutes due to history of PCN allergy. After about 10 minutes, client returned with complaint of itching, flushing in face and hyperventilating. LPH assisted and laid patient down and elevated her legs. Pulse was 200 and breathing difficulties increased. EPI 0.5 given IM in thigh by nurse. Patient felt significantly better after one dose of EPI and breathing was almost back to normal. Patient transported by EMS.

Rash and itchiness started at around 10 PM on 1/7/2021. Rash described as "traveling across chest" and similar to what happens when she takes prednisone. Stated she has not taken prednisone for over a year. Client stated that rash did not appear anywhere else on her body. Itchiness was experienced on chest and arms only. Client also stated symptoms of fatigue, muscle weakness, and "super sore arm" that lasted from day of vaccination until 1/10/2021. Client also reported a headache that lasted 2 day after vaccination. She contacted her provider on 1/7/2021 and, per provider's instruction, took 1 tablet of Benadryl. Client was unsure of dosage of Benadryl. On the morning of 1/8/2021 she took a second tablet of Benadryl and reported that her rash resolved after the second dose of Benadryl. Her itchiness resolved the morning of 1/9/2021. Stated that her Doctor did not specify a diagnosis but did state to hold off on the second dose of this vaccine.

Systemic: Rash (other than injection site)-Medium, Systemic: Exhaustion-Mild, Systemic: Fever-Medium, Systemic: Generalized Body Aches -Severe, Systemic: Headache-Severe; symptoms lasted 1 day

I had no pain or discomfort immediately after the shot, during the 15 minute observation period, or an hour after. About 2 hours later (at 2:00 pm), I developed some hives around my neck, and the part of my arm near the injection site started to become more and more sore. I applied Benadryl to the hives and they went away. However, my left arm hurt to touch or move, much worse than any vaccine I've had before (flu, HPV). That night, I had trouble sleeping because the discomfort because of the sore left arm. I also could not sleep because of chest tightness and pressure, which worsened when lying down, but that pressure did not progress to the point of wheezing (I have asthma). Sleeping on my back was the only option because sleeping on either side was painful, which worsened the chest pressure. When I woke up, my fingers felt stiff and I had joint pain in both hands, more so in my left hand. Throughout this, I did not see any redness on my arm. This day it was agonizing to move my left arm. I could pick things up, but raising it above my the height of my waist or twisting it behind my back to get on my backpack was near impossible and was avoided. The chest pressure went away. The second night, I still had trouble sleeping, but the pain was much better, although I still could not sleep on my side. When I woke up, I had no more joint pain. That day, although the left arm was still sore, I could move it pretty well. If I made too strong of a movement, it felt like the injection site would tense up, the pain would slowly ramp up, and then diminish. That afternoon, at 4:30, I was able to play tennis with minimal soreness, and that night, I was able to sleep on my side. The following day, I had a cough, but that might have been because I didn't dress for the weather, and experiences close to zero soreness.

Erythema multiforme minor with skin lesions on hands and feet starting 10 days after receiving the vaccine followed by hip, knee and lower back pain

8 days after injection left arm turned red around the site of injection and began to grow and get worse, 9 days after it became hot, itchy, and the arm got firm in the spot where it was red. Took Benadryl thinking it was an allergic reaction and it did not do anything, continued to grow.

7-days following vaccination, employee developed redness and swelling of injection site, denies itching, or pain to area

Recently noticed that injection site is swollen and itchy and red, as well as multiple red patches surrounding the injection site

10:02am client reported right eye twitching and lightheadedness. Then noted rash on hands, nausea, and DOB. 10:05am 0.5ml 1:1000 Aq Epinephrine given in left thigh as paramedics arrived on scene., vitals taken. 10:18am Oxygen started per face mask and client transported to hospital at 10:20am

For my first dose, I had quite mild symptoms. I woke up with a migraine the next morning and some soreness at the injection site. Both of these lasted a couple of days and were able to be treated with over-the-counter pain medicine. My second dose, however, provided much worse flu-like symptoms. About 12 hours after I received the injection, I was woken with a migraine much worse than the first, nausea, chills, joint pain, sore throat, and fatigue. While the chills and nausea subsided, the other symptoms persisted until the next day. As of today (2/4/21), all symptoms are gone except for soreness at the injection site.

1/24/21 morning, 10 days after getting 1st dose of vaccination, broke out in hives in several places on both feet. The next day more hives appeared on knees and the ones on feet got worse. The day after that hives also appeared on hips and elbows. On 1/27 went to Doctor's office and received steroid shot to stop the allergic reaction. Took 5-7 days for all the hives to disappear. Based on the allergic reaction to the first dose, doctor recommended not taking the second dose as reaction could be more severe and/or have an asthma attack.

Patient received 2nd COVID vaccine Moderna 2/3/21 to the left arm and now having redness that is spreading, hot, itchy, and hard lump to injection site that is also very painful. Patient did outline the redness when started to monitor the increase of size that now reporting getting worse. Has pictures of the arm with the progress. Had applied topical benadryl cream to site without improvement. Concerned about cellulitis to the area. Having bad headache, body aches and chills. Reports that she did have the body aches and headache as side effect with the first vaccine but recovered after a day or so. Scheduled appointment in office to further evaluation

on second shot i got alot of symptoms/bad reactions. nausea, feeling light headed, aches in my body everywhere, a nose bleed, high fevers (102.9). My left arm from where i got the injection was really swollen after two days prior from getting the vaccine.

18 y.o. female with a penicillin allergy received Moderna COVID vaccine at a distribution site. After 40 minutes began to develop to develop pruritus, flushing of the face, and started hyperventilating. Nurses laid her on her back and elevated her legs. Her pulse was 200 and 0.5 mg of epinephrine IM was administrated to her thigh. She began to feel better and was transported by EMS.

I was fine when I went to bed the night I got the shot 02/05/2021 then the next morning I woke up with hives covering the arm opposite of the shot, and my knees had small spots as well it went away an hour later and then come back about 2 hours later and I was covered again all down my legs and arms were covered in hives. I also had body ache and a horrible headache the whole day. I took Benadryl for the hives and it seemed to have worked. The adverse events happened on 02/06/2021

Swelling, pain and redness for the first few days. Then led to an anaphylactic reaction and had to get IM steroids and Benadryl.

Patient found on floor pale, diaphoretic, unconscious approximately 3 minutes after receiving vaccine. patient became suddenly alert on first attempt to rouse. Noted 5 cm reddened area on forehead which was assumedto be a result of hitting her head on the nearby plastic trash bin. Pt Ox3 upon awakening. She was aware that she had lost conscousness during a syncopal episode. She denied smilar reactions in the past. Rapid Response called. Ice applies, 11:35 BP 104/66, HR 79, SpO2 99%. Assisted to a sitting position. 11:40am BP 108/70, HR 72, SpO2 99% RR arrived. Pt assisted to WC then to cot. Denied Further dizziness. To ED via security vehicle, accompanied by security officer and MA from clinic.

H/o sulfa allergy, monitored for 40 minutes, felt well. 15-20 min later called clinic, felt like throat was closing, advised to go to ER, she arrived in ER while I was on the phone with her. Subsequently developed itching. Was treated in ED with Epi-pen, Benadryl and steroids. Home after observation and improvement. Spoke with patient ( at home) next morning, doing well. Has appointment scheduled with allergist.

immediate rash, redness over entire body, shortness of breath, swelling and tightening of chest, dizziness, extreme nausea and faintness,? spacey ? feeling all within 30 seconds of vaccine. administered benadryl 50 IM by pharmacists immediately. dizziness continued 3 hours. 911 called paramedics arrived and assessed. allowed to go home with parents.

A red rash is appearing near the spot where I got my vaccine. It is itchy. I believe it is called ?Covid arm.? It feels a little warm. I noticed that it started itching and then I looked at it and it was red. It happened aroung 8:50-9:00 while I was working out.

18 year old female came to hospital for chest tightness, choking sensation in the throat as well as skin rash after receiving COVID-19 vaccine. Initially patient presented to emergency room but subsequently discharged on steroid and Benadryl. Patient had continued episode of choking as well as chest tightness and skin rash for that patient came back to emergency room. Patient had received total 5 doses of epi pen because of that. Patient was admitted to medical floor initially. Patient was given steroid, Benadryl and famotidine. Patient had 2 episodes on medical floor where she started feeling choking and chest tightness. Patient remained stable hemodynamically except for tachycardia. Patient was transferred to intensive care unit for close monitoring. D-dimer was negative, urine tox screen was negative, COVID-19 PCR was negative, EKG showed sinus tachycardia. CT scan of neck was negative. We had discussed that patient has remained stable hemodynamically in the intensive care unit and discharge planning was discussed and patient was getting ready to be discharged when she had another episode where after drinking soda patient started having that choking sensation, patient remained stable hemodynamically during this episode as well. Her symptoms had improved. She looked visibly anxious. Patient was monitored in the ICU for few more hours and patient remained stable. I checked back on her and she was able to eat her lunch. She probably had a allergic reaction to more than a vaccine but subsequent episodes are thought to be secondary to anxiety attack given situation.

Redness and swelling around the vaccine site 7 days after vaccine administration. Conservative monitoring.

post vaccine: sudden onset of dizziness, vertigo sensation with nausea, witnessed near-syncopal event, "everything going black in color" per patient, skin clammy and pale/warm and diaphoretic, spinning sensation, bradycardia and mild hypotension with abnormal cardiac rhythm- EMS called by on-site paramedic at 12:55 12:50 HR 42, BP 90/64, SaO2 97, Resp 18, sinus bradycardia. GCS=15 1306= HR 105, BP 102/64, SaO2 98, Resp 16, Sinus tachycardia, GCS=15 13:16 blood glucose 175 mg/dL No medications required

appx 12 hours after receiving her first Moderna Covid19 vaccine, She developed a red itchy hives like rash across her torso that spread over her entire body within 2-3 hours, at which point she took two oral antihistamines (Loratadine) and applied some topical hydrocortisone cream to her body. The scary rash subsided overnight (a week ago) and she hasn't experienced any further reaction.

On Saturday March 13, 2021 she developed a several cm erythematous, raised rash overlying the site where she had previously received her COVID-19 vaccination. In addition on March 14, 2021 a Sunday she experienced raised red areas of the skin which were pruritic on her knees, hands and legs. These were also present on her hands and elbows per report. She has been taking Claritin/loratadine 24 hour tablet once daily and 25 mg Benadryl each evening. Over the past 2-3 days she feels her symptoms have now turned the corner and are improving.

Difficulty swallowing; Shortness of breath; Big rash; A spontaneous report was received from a consumer concerning a 18-year old female patient who received Moderna's COVID-19 vaccine (mRNA-1273) and experienced events shortness of breath, difficulty breathing, tightness on the throat, difficulty swallowing, big rash in forehead and under her eyes. The patient's medical history was not provided. Concomitant medications reported were birth control pill for drug use for unknown indication. On 9 Mar 2021, prior to the onset of the events the patient received their first of two planned doses of mRNA-1273 (lot/batch: unknown) intramuscularly for prophylaxis of COVID-19 infection. On 9 Mar 2021, the patient experienced the event shortness of breath, difficulty breathing, tightness on the throat, difficulty swallowing, big rash in forehead and under her eyes. Treatment details included went to the emergency room (ER), epinephrine, diphenhydramine, and dexamethasone.. Action taken with mRNA-1273 in response to the events was not reported. Developed a painful hives on bottoms of feet, hives later traveled to tops of feet, legs, hands and arms. Pain in feet lasting 5-7 days causing pain with pressure/walking. Hives/rash at the site of the injection also appeared at the same time (day 7 after vaccine) Evaluated by Clinic allergy as well as emergency dept. Felt to be an adverse reaction to COVID vaccine- recommended not receiving 2nd dose.

Patient described to me that she received the 1st dose on 1/4/21. She noticed a reaction about 10 days later, and it presented as a red rash on her body. This rash did get worse after the 2nd dose of the Covid vaccine on 2/1/21. The rash is red and raised and does itch. It is all on 4 extremities, her abdomen, and her back. It is on her face, and on her scalp. She did a telehealth visit with an MD who prescribed her steroid cream. This did not help, and she has a dermatology appt on 3/30/21. I wish there was a way I could upload pictures because what I am describing does not do it justice. Her entire body is covered with this rash, and it is causing her a lot of discomfort. She did state the red, raised areas bleed if she scratches them, which she avoids trying to do.

Found on floor pale, diaphoretic, unconscious approximately 3 minutes. Suddenly became alert on first attempt to rouse. Noted 5 cm reddened area on forehead as result of fall and hitting a plastic trash can. Orient X3 and aware that she had lost consciousness during syncopal episode. denies similar reactions in the past. Rapid Response called. Ice pack to forehead. 1135 BP 104/66 HR 66 SpO2 99% Assisted to sitting 1140 BP 108/70 HR 72 SpO2 99% Sent to ED.

After 30 minutes I became extremely tired, this lasted for 24hrs. I was very fatigued. The injection site also became red and swollen. The surrounding skin started to peel. This started after 3 hours and is still occurring.

Right eye twinching; lightheadedness; rash on hands; Nausea; DOB; This case was received via an unknown source (no reference has been entered for a health authority or license partner) on 10-Feb-2021 and was forwarded to Moderna on 11-Feb-2021. This spontaneous case was reported by a nurse (subsequently medically confirmed) and describes the occurrence of BLEPHAROSPASM (Right eye twinching), DIZZINESS (lightheadedness), RASH (rash on hands), NAUSEA (Nausea) and DYSPNOEA (DOB) in an 18-year-old female patient who received mRNA-1273 (Moderna COVID-19 Vaccine) (batch no. 037K20A) for COVID-19 vaccination. Concurrent medical conditions included Penicillin allergy since January 2021. On 29-Jan-2021, the patient received first dose of mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) 1 dosage form. On 29-Jan-2021, the patient experienced BLEPHAROSPASM (Right eye twinching) (seriousness criterion hospitalization), DIZZINESS (lightheadedness) (seriousness criterion hospitalization), RASH (rash on hands) (seriousness criterion hospitalization), NAUSEA (Nausea) (seriousness criterion hospitalization) and DYSPNOEA (DOB) (seriousness criterion hospitalization). At the time of the report, BLEPHAROSPASM (Right eye twinching), DIZZINESS (lightheadedness), RASH (rash on hands), NAUSEA (Nausea) and DYSPNOEA (DOB) outcome was unknown. Hives on chest/neck, armpit area. Took a Benadryl and mostly went away. Hives reappeared 48 hours later--chest, arms, stomach, back. Gave Benadryl again. No additional symptoms.

Pt had 2nd dose of covid vaccine moderna 1725. At 1735 pt complain of Dizziness and a little numbness on the right side. VS done BP 136/86, RR 16, HR 86, O2 sat 99% at RA. At 0540 Rash appeared on pt chest , PA notified, PA assess pt And rash begin to appear on pt neck and face, Verbal order of Benadryl 50 mg IM ordered by PA. Verbal orders from PA conveyed through department-approved policies and procedures. Given 50 mg Benadryl IM on pt right deltoid. Pt tolerated it well. 911 activated at 1740. PT BP 122/95, HR, 116, RR 16, O2 sat of 98 %. . 1745 911 on scene BP of 140/89 , HR 94, RR 17, O2 sat 98% at 2lpm via nasal canula. At 1750 Ambulance came and pt left with stable condition , rash getting better pt is alert oriented x4. Left clinic at 1750 with rig pt will go to ER. BP of 143/102, HR 83, RR 16 O2 sat 98% .

About 72 hours after her 2nd Moderna vaccine, patient developed an uncommon condition known as Lipschutz ulcers. It is a non sexually transmitted condition characterized by the rapid onset of painful, necrotic ulcerations of the vulva or lower vagina. Pt presented with moderate unilateral labial swelling, several vulvar ulcers noted to inner labia minora with yellow/bloody discharge. Some ulcers with necrotic appearing tissue. Extremely tender to touch. Pt without hx of EBV infection.

Splotchy, red, macular, blanchable rash to upper left arm. Non tender. No ulcers, blisters, swelling or bleeding. Irregular, but clear borders.

About 4 hours after getting the injection she reported pain in her arm , right side of chest, and passed out. She was trying a dress on in a store and a woman was adjusting it and she all of a sudden said mom I don?t feel good I think I?m going to pass out. She reports everything going black. She go so pale almost blue and passed out immediately she started sweating and had a slight rash on her back. EMS was called she was alert before they arrived they checked vitals and all were normal. The rash went away within a day.

injection site (where they put the vaccine in my arms) started itching profusely and getting a big red circle about 2 days ago. the itchiness has only increased. the area is hot, hard, and red, right where the needle went.

Had four hives/welts lined vertically along her spine. About 1/4 to 3/4 inches in size. Length of all four welts were about 1 foot in length. One welt was blistered and oozing. Looked liked potential chicken pox or shingles?

She is in between doctors right now and does not feel like we need to contact him. Will talk to her new doctor next time she is seen

Itchiness that comes in waves: sides and nape of neck, palms, left armpit, undersides of forearms up through thumbs, backs of hands up through fingers. Itching until red and sore and does not cease after use of body lotion as an eczema breakout would. Significant itchiness during evening of vaccination day, mild on day after. Self-treated with two antihistamine tablets in the evening and one in the morning.

Patient reported feeling itchy and had red rash on back of neck. Pt immediately given 50mg of oral liquid diphenhydramine. Pt then reported to us her rash was now showing on the left side of her abdomen. Epipen given and patient felt better for and itching subsided. Pt began to itch again, so second Epipen was given and called 911. Pt was transported to the hospital.

Patient received the first dose of Moderna vaccine on 2/5/21. Patient reports waking up the following morning (2/6/21) with a rash on her chest. The rash progressively worsened/spread over the weekend to her back, arms, stomach and legs. patient reports calling the on call physician for her medical practice on Sunday 2/7/21 to report the rash. Advised to schedule a visit the following day for evaluation and to go to ER if experiencing high fever, difficulty breathing, etc. Patient reports she called to schedule a visit and was seen on Tuesday, 2/9/21. She was advised against receiving the second dose of Moderna vaccine and prescribed Prednisone taper. Physician's note indicates the rash covered the entire

apprx. 6 days post vaccination pt reports 2 inch diameter red/pink rash at injection site. "Covid Arm": Roundish, inflamed area on injection site. Area is hard, warm, and red. No pain or itchiness. Noticed on Monday (4/26) morning, still present 4/30 (8 days after injection).

When I went to the patient, she was crying and hyperventilating. I told her to put her head down and take deep breaths. I checked on pt in 15 min, was doing better but had burning sensation at injection site. Kept pt at pharmacy for 30 minutes.

Developed bilateral ankle pain on 4/28 which progressed to significant ankle pain (difficulty bearing weight) and rashes to both ankles, now spreading up both legs on 4/30.

Multiple rashes developed on the upper torso that later spread to other areas touched. Rashes blistered. Treated in urgent care on 4/18/2021. Prescribed oral steroid and topical anti-fungal powder. Rashes have cleared, but severe scarring remains.

Hive like rash on elbows and knees. Began 9 days after injection on elbows. Raised and itchy. 10th day had spread to both knees and some finger joints.

Patient called and stated itching followed by redness/hives developed around her right elbow the morning after her vaccination. An 18 year old female, with no reported history of allergies, presented to store on May 3, 2021 for her second COVID-19 vaccine. Approximately 24 hours after vaccination, she developed a headache. Approximately 48 hours after vaccination, she developed a rash that spread from the neck to the legs. She went to Hospital Emergency Room. In the ER she was given Banophen (Benadryl) and famotidine. Later that same day, the symptoms resolved. At approximately 9am on May 5, 2021, the rash returned and spread to the palms of the hands and the face. At approximately 11:20 am, the patient took Banophen and famotidine. She called the pharmacy around noon for advise, as the rash was still present. Patient was advised to call her primary care physician.

received her 2nd dose and within 3 minutes she complained of Left Arm pain.....by 5 minutes it was moderate pain and her left antecubital area was reddened, swollen with a hard area the size of a quarter. Within 10 minutes, her arm was in severe pain and her left hand was cool and mottled. She was transported to the ER and was diagnosed with "lymphadenopathy reaction" and was sent home. About 12 hours later, she woke up at 3am with shallow breathing and facial swelling, she was transported to Emergency Room and was treated for an "allergic reaction to vaccine" and released.

8 days after injection I had a delayed hypersensitivity reaction (?covid vaccine arm?), symptoms included firmness and a red patch on my upper arm near the injection site. The following morning, I woke up feeling well with the rash still on my arm (no change). 2 hours later I began feeling dizzy and fainted. I was unconscious for 5 to 10 seconds. When I woke up, the rash had vanished and the firmness was gone. No signs of the reaction were present after waking.

The first day, I had slight arm soreness with nothing more. When I woke up on day two, I noticed my arm had swelled more than usual (basing off my previous dose in my right arm) and was extremely hot, tender, and had two circular red patches. That night, the redness and swelling had spread into a medium sized circle, and began to itch and still be very hot upon touch. Upon looking later that night, at approximately 11pm EST, the swelling and redness had gotten larger. On day three, the redness only got worse, and is slowly spread more over the area, with intense itching, but no longer is as sore as it was the day prior. I have iced the area, moved my arm around more to help the swelling go down, taken a hot shower, and about 600mg of Ibuprofen each day since the second dose. The only outcome to those treatments has been a significant decrease in pain level, but the swelling and redness still continues. This has all been over about a 48-72 hour time period

Febrile beginning at 1600, 100.5 degrees, peaking at 2100, 104degrees, accompanied with tightness in chest with mild difficulty breathing and extreme muscle weakness most notably in extremities, bilaterally. Needed assistance to stand and could not hold a pencil to write. Also numbness and tingling in hands and fingers. Face slightly swollen and red. 400 mg Ibuprofen was administered at 1530. Pushed fluids as soon as fever began. Cooling measures provided, cool shower, cold compress, change of clothes to light pajamas. We contacted our Insurance 24 hour Nurse line at approximately 2100 when symptoms peaked. The RN on staff instructed us to seek immediate medical care. We attempted to find an open Urgent Care unsuccessfully and tried to go to an ER but they were full. Fever began to come down around 2230, so we returned home. Continued with ibuprofen every six hours and cooling measures.

Chills, fever, Red rash on entire face, chest , arms and back. The rash appeared 2 days after (05/14) to present day (05/16). I was given Methylprednisolone to stop my body?s reaction to the shot.

Patient broke out in hives in sizes ranging from a dime to a half dollar coin on arms, legs, hands, and feet. The patient visited a local urgent care where the cause was not able to be identified. The doctor prescribed the patient 6 doses of Prednisone.

Erythema multiforme rash developed within 2 weeks of the vaccine and progressed over the next 2 weeks. Patient had fatigue, abdominal pain, nausea, and vomiting about a week after the first spots appeared. Prednisone x 3 days was initially prescribed with no progression but no resolution of the rash. After patient came off steroids, rash progressed and was then restarted. After one dose, patient started vomiting repeatedly and was seen in the ED. Prednisone was discontinued and patient was started on topical steroids and hydroxyzine with improvement.

Pericarditis and pericardial effusion with pulmonary effusion, fever, tachycardia and elevated inflammatory markers, knee pain, rash

She has a small reaction after the first shot. After the 2nd shot, she had the following reaction: Injection site swelled up the size of a baseball and was very sore and hot to the touch, broke out in a rash around her lips and in her mouth, lips started to swell and felt like they were "vibrating". Has lost her appetite and lost taste for about a week following the injection.

*Note: I have never had allergies before my covid vaccine. I woke up the following day with red, itchy hives on my face which soon spread to my hands. These hives lasted for about 5 days. Since my covid vaccine, I have developed anaphylactic allergies to random things including my birth control which I have been on for two years. I also have discovered an allergy to something in my facial cleanser (maybe coconuts).

"Patient received 1st dose of Moderna at 11:06 am. Patient was able to walk to observation area and at 11:10am, observation nurse noticed patient's face losing color and she was able to catch him and help him to the ground without him hitting his head. Charge nurse was notified and vitals taken at 11:13am B/P: 120/60, Temp.:97.9, Resp.: 17, HR: 60, O2: 98%. Patient was able to sit up in chair on his own. After a few minutes , patient said he felt nauseous and vomited. Water was given to patient and he also drank some Gatorade. At 11:20 am patient went to sit outside with the charge nurse, he was able to walk steadily and without incident. After waiting a total of 30 minutes patient said he felt "much better" and was able to leave with his parent who was driving him home. Patient was able to stand and walk steadily without assistance" Charge RN

A day after vaccine, extreme nausea, vomiting uncontrollably, swelling and redness at injection site, arm looked like a golf ball, no appetite, dizzy, extreme joint and muscle pain, fatigue. Was diagnosed as an allergic reaction to vaccine at urgent, given liquid tylenol to cut down fever and joint/muscle pain, anti-nausea pills.

Case began filling ill several hours after immunization. She experienced headache, chills, feverishness, joint pain in legs, nausea, vomited once, One July 2 about 1600, she experience purple spots on her face and neck.

The very next morning after the vaccination around 7:00 AM , my right arm was hurting, it was not red, or there wasn't any bruising. I had tightness in my throat, it was pretty much like an anaphylactic shock is how the doctors described it. I had white bumps, like welts, on the back of my throat and tongue. And the roof of my mouth and my tongue, my lips everything in my mouth was swollen. My eyes were almost swollen shut. On 12/27/2021, I had a doctor video visit. He advised me to take Benadryl, and Tylenol, I used my albuterol inhaler several times because I couldn't breath. So around 3:00 pm I went to the Emergency Room because I was still having a hard time breathing and the swelling wasn't going down. The doctor immediately gave me an Epi pen. and told me i had an allergic reaction to the vaccination. The doctor advised me that it was up to me to take the 2nd vaccination . He said it was up to me.

Moderna Covid-19 vaccine administered 5/25/21 5/26/21: Experienced the following in injection arm: - Large red spot around the injection site - Itchy - Arm very sore and hurts to move - From pain scale of 1-10 (10 being the worst) reported it at a 7 - Upper arm is swollen - Took Tylenol and applied ice to the injection site; Called off of work. 5/28/21: Swelling in left arm started to subside, the itching stopped and patient could move arm around. Spot was still "warm to the touch" and the redness around the injection site has spread. No leg swelling or calf pain. Picture was emailed in via MyChart. 6/22/21: HIVES - Started on left leg 6/12 and progressed since then; now spreading to upper/inner thigh and showing on right leg. Hives red, itchy. Similar to previous allergic reactions. Picture sent via MyChart. LEG SPASMS - Started in the left leg on 6/7 and have continued; now altering legs. Describes as "charlie horses" in calves typically at night. PERIOD - was late but did have it and now bleeding stopped. Patient has also had intermittent tension headaches since getting the vaccine. Takes Tylenol, rests and these resolve. There is no shortness of breath, chest pain. No facial lip or tongue swelling.

Pt has persistent right shoulder pain s/p her 1st Moderna shot in March 2021. She had no problems with her 2nd shot (in her left shoulder), in April.

Patient started to develop full body rash and itching the day after the shot. She took Benadryl over the counter until it was not keeping her symptoms under control. On 07/21/2021, she went to her doctor and was given steroids to control her allergic reaction. Dr. told her to not receive the second dose.

Patient stated that the day after the first shot that her body odor smells like a rotten egg. A week before she received the second shot she stated that she started to smell it in her bodily fluids. She stated that she still has that smell and she can taste it in certain foods. After second shot she stated that her face broke out in a severe hormonal acne. She went to a dermatologist and has been treating the acne with medication.

Arm hurt badly about 10 hours after shot. Felt super weak and tired too the whole day and the next day. A rash started on my neck and traveled down to my collar bone area by day 4

Patient received Moderna vaccine, Wednesday 12/30. On Saturday 1/3/2021 patient felt pressure/tightness in lower extremity. When patient touched area, a noticeable ball was felt under the skin, tender to the touch and warm. Patient went into urgent care on Monday 1/4/2021 with a confirmed dx of a superficial blood clot. Unknown etiology of whether this is from current birth control or the COVID19 Moderna vaccine.

Moderna COVID-19 Vaccine EUA Very anxious prior to receiving first dose of Moderna Covid Vaccine. One hour after receiving vaccine complaint of itchy hands and felt as if "ants were crawling all over her". She said has an allergy to Latex and was previously wearing Lavender Nitrile gloves. No hives or erythema on exam, some mild erythema to area after scratching hands with nails, no raised wheals. Sensation of pruritis spread to back and neck without urticaria. Diphenhydramine 50mg was administered. Approximately 30 minutes patient complained of nausea and was give Ondansetron 4mg which alleviated nausea. Pruritis resolved but approx 30 min patient began to feel tired, dizzy and lightheaded. Vitals were taken and orthostatic blood pressures were performed. All vitals were normal, oxygen saturation was normal and injection site was without erythema and no evidence of urticaria. Physical exam was also performed and no abnormalities to heart or lungs were auscultated.

Pt. reports that she noticed her left arm where she received Moderna COVID- 19 Vaccine EUA was red, swollen and itching this morning (01/06/2021) at approximately 7:45 AM. Pt. also states that she started taking Bactrim on 01/05/2021 for a UTI and she took only one dose prior to the event. Reports taking Amoxicillian for stye last week and was switched to Bactrim on Monday for a UTI but did not take first dose of Bactrim until Tuesday 01/05/2021. Pt. also reports all over itching including legs and face it red and blotchy on both cheeks that also began today. Pt. came into clinic on 01/06/2021 at 1:54 PM for visual evaluation of arm. Upper outer left arm (left deltoid area) has red rash 55MM Length & 30 MM Wide Area is also warm to touch. Pt. used cool compress for the redness, swelling and Benadryl for the itching.Pt. reports that swelling and redness has improved but itching continues along with aching. Reports a numb feeling in the area of the shot on left deltoid. Pt. also reports left axillary tenderness but states that started shortly after vaccination. Patient's mother notified primary care provider due to current use of Bactrim but no actual visit to doctor was made. Pt. instructed to continue to monitor for side effects and to contact HD and primary healthcare provider if symptoms worsen.

Multiple times we asked her if her lips or throat was swelling. She said no. She went to urgent care-they sent her home with advice to take Benadryl or APAP.

injection site has been itchy since i got the shot. i noticed about four days ago that my lymph nodes in my left armpit were super tender. about six days after injection left neck lymph nodes became extremely swollen and tender. on day seven i noticed a rash all over my upper left arm from top of shoulder to about half way down my upper arm. extremely red and super itchy.

After I recieved the Covid-19 Moderna vaccine I began to feel ill. I became pale, had fatigue, headache, stomach ache, and the arm i recieved the vaccine in was sore to the point I couldn't hardly move it. These side effects lasted until 12/25/2020. I had no fever and after the 25th I felt fine.

My arm was extremely sore and red with a large bump..it then get better starting January 13 2021 I noticed the bump and red spot coming back it is now very large and swollen itchy and sore i have a headache and am very fatigue a week after

Rash all over face, chest, neck, and back which is continually getting worse. The rash is warm to the touch and is beginning to itch.

Intensification of joint pain and subsiding of rash until 1/12, when right hand and wrist were excruciatingly painful, then 1/13 was excruciatingly painful. Intensity of arthralgias has lessened since starting Prednisone taper 1/12.

Swollen lymph node above my left clavicle, tender and warm. First felt when I woke up around 8:00 AM on 1/17/21.

pt developed large rash on arms and torso and elevated blood pressure and pulse 138-110 pulse 100 - pt given 50 mg of diphenhydramine at 10:35 am by 11:05 rash was fading and 90% clear.

One week after the shot (1-14-2021) Patient (19 y.o.)reported side pain and appeared constipated, Laxatives given along with Tylenol, on further assessment Patient was noted to have left leg redness and abdominal fullness. Dr. was updated and we had orders for close monitoring, the next day when she got up, her leg appeared better, and she had passed a small BM, but by lunch she had developed significant pain and edema in her left leg, and the color of her leg was reddened again. She was sent to the emergency room with her symptoms. She was admitted back to our facility yesterday, her diagnoses included Acute provoked left external illiac, femoral, popliteal, and peroneal DVT. Elevated Factor II levels, Elevated APC resistant, May-Thurner Syndrome, history of developmental disabilities, fecal impaction and urinary retention - suspected related to her fecal impaction. Vascular surgery was consulted, and pt. was started on a heparin drip, and mechanical thrombectomy was needed for both legs due to multiple clots. She was started on Eliquis and Plavix, and thigh high compression stockings were ordered, ace wraps being used until these are supplied. Her Fecal impaction was addressed also and the urinary retention resolved.

For two days after injection, I simply had regular, dull, pain in my arm from the injection. This got better and I had no other symptoms. One week from the date of my vaccine, I woke up with a swollen and red arm below the injection site. It was itchy and painful. This has persisted for two days and I saw a physician for it. We are currently monitoring the site for possible cellulitis.

Approximately 15-20 minutes after vaccination patient developed weakness in her limbs. BP and glucose were normal. Approximately 10 minutes after the onset of symptoms the patient developed bright red rash on the face, increased breath rate, and swelling of the lips and tongue. She was having difficulty talking due to shortness of breath and swelling in the tongue. 911 was called and epinephrine 0.3mg IM was administered as well as diphenhydramine 100mg orally. Her rash improved fairly quickly, swelling went down a small amount. She stated breathing still felt labored and she was feeling like her heart was racing but she was also tired (likely due to medications administered). She was still having difficulty talking, taking short breaths between almost each word. Paramedics arrived approximately 12 minutes after epinephrine was administered. They attempted to walk her to the ambulance per her request, however she was unable to feel her legs. She was placed on a stretcher and into the ambulance. The patient stated she was given additional epinephrine in the ambulance as her condition had worsened by the time they got her in there. She was taken to the ER and given additional diphenhydramine and was observed until approximately midnight, when she said she was discharged. Follow up was completed the following day, and she had improved dramatically. She was still feeling a bit weak, but no longer had any issues breathing.

Slight soreness of injection site until today. There is a 1.5 X 2 in raised, red triangular area around the injection site. It is warm and painful; however, the patient says it is not getting more painful. The actually injection point as a raised, smooth, moderately dark pink wheal about 4 X 4 mm. No drainage at site. The Wheal reminds me of an old smallpox vaccine scar.

A bad arm pain, fatigue and nausea. 2 days after, itchy, vein looked like inflamed, lump on right clavicle started as a size of a pea but has grown bigger to the size of a walnut, hard and painful, pain up neck and down right arm , tingling on fingers

Vaccine clinic held on 1/21/21. Vaccination provided by pharmacist. Patient received a vaccination at this clinic. On 1/22/21 patient reported to her supervisor she went to the doctor because her injection site became hard, hot and red and was given an antibiotic for an possible infection.

Vaccine was administered into the subacromial space/joint of right shoulder instead of deltoid muscle. Patient presents on 1/27/2021 after onset of pain, decreased ROM ongoing since administration. I was able to visualize ecchymotic area where vaccine was administered and patient has photo evidence. The physician responsible for the vaccine administration needs to be retrained on where to give deltoid muscle IM injections and to avoid the extremities where recent surgeries occurred (patient had right shoulder labrum repair in October 2020)

Skin rash on arms, legs, hands, ankles, feet, thighs. It is red, splotchy, and itchy. Discomfort on the joints when bent where the spots are.

Received vaccine 2/2/21 at 9 PM, began feeling major pain and swelling at injection site at around 12 AM. Took 500 mg Tylenol On 2/3/21 around 3 PM began feeling extremely fatigued and had a migraine. At around 5 PM began feeling nauseous and dizzy and at around 7 PM began to have body aches. Took temperature later at 11 PM and got 100.8 F. Believe I had above 101 earlier in the day. Took 1000 mg tylenol before bed. Throughout the night woke up multiple times drenched in sweat and had hot and cold chills. Arm pain is getting better though.

Thumbprint sized Hive on forehead for more than 24 hours. Hive size of two quarters on back of right thigh. Itchy all over. Site of injection itchy.

Patient is a 19yo female presenting to the ED with chest pain. Patient received her second Moderna vaccine two days. Since then she has been having symptoms of chills and myalgias. This morning she had severe chest pain in the middle of her chest. It made it difficult to breathe. Patient had no side effects from 1st dose of vaccine and denies any symptoms prior to receiving 2nd dose.

Sore throat (difficulty swallowing), swollen glands, joint pain, low grade fever (99.8), sore arm, for 7 days. Day 9 delayed COVID arm with 8 x 8cm outer ring, and inner bullseye of 4cm x 3cm hot to touch, pain on site, swollen . Returned low grade fever on Day 15.

The site of the injection felt itchy for days following the injection. On February 12, I noticed an itchy bump on my right hip. The next morning, there were itchy, hard bumps on my feet and right ring finger. The bumps on my feet have become swollen and warm and seem to be growing larger, especially today on February 14. Two small bumps have also developed on my hands. I am not sure whether this is related to the vaccine, but it has become painful to walk.

On 01/19/2021, 4 days after receiving the first dose of the Moderna COVID-19 vaccine, a small, splotchy red patch appeared on my left arm beneath where I received the vaccine. Over the next few days, the rash became increasingly red and square-shaped. By the time I received my second dose of the Moderna COVID-19 vaccine on 02/12/2021, the rash had mostly subsided with some general redness remaining. Then on 02/13/2021, the rash reappeared as a milder and smaller version of the original rash. No treatment has been used, and I have not seen a medical professional about the rash. As of 02/15/2021, the rash is still present.

Onset of uterine cramps about 45 minutes after injection, with a pain level of 4/10. Have continued non stop since injection. Very tightly concentrated cramps at cervix. Pain level ranges from 2/10 to 7/10 every day. Received vaccine 14 days ago and cramps have not gone away. No other reproduction illnesses currently, and uterine cramps are not normally experienced during any other times, or if so range from a 1/10 to 3/10 during menstrual cycle. Skyla IUD has been inside since 8/21/2019. Dark purple bruising at injection site approximately 2inches by 3inch. Bruising is faintly present and still sore to touch as of 2/17/2021

Full body Hives started 2 days after receiving vaccine, doctor determined vaccine was the cause. Extremely itchy patches in all of the warm places on the body. Was prescribed with an anti-itch pill and an antihistamine.

INJECTION SITE NOT PAINFUL OR BRUISED BUT ARM WAS VERY STIFF AND SORE FOR DAYS. ** high fever and pain in arm which was a normal reaction. Took ibuprofen and applied heat for pain..

Broke out in hives, ER gave me Epi and Benadryl immediately. Symptoms began to resolve almost immediately.

After the patient had waited her 15 minutes she got up to leave; on the way to the door she said she began to feel weak, have slow reaction, dizziness, and felt warm to the touch. She cam back in side the room and we had a nurse assess her, her BP and heart rate were a little elevated. We gave her some water and had her sit for some extra time while we talked to her. She has a child hood trauma regarding vaccine and had just had a major panic attack before and during her vaccination. We were all pretty sure she had just worked her self up and was not feeling well because of that and she said as much herself. After about 15-20 more minutes her BP and heart rate were back to normal and although still feeling a little weak she was ready to go home with her mother.

Pt received vaccine on 2/25/21. Presented to emergency department on 2/28/21 with c/o hives (generalized) and arm burning sensation starting 2/27/21. Pt received famotidine, methylprednisolone, and diphenhydramine in the ED; was discharged to home.

Vaccine area is warm to the touch, large and round red discoloration and it itches even though there is no pain when touched.

Bruises all over legs. Bruising continues today Monday March 8 with new bruises from March 7 from slight pressure on knees. Concerned about immune thrombocytopenia reports being linked to the vaccine.

Angioedema, Urticaria, hives, shortness of breath Developed facial swelling approx 3h after receipt of 1st-dose Moderna COVID-19 vaccine. Subsequently developed hives and trouble breathing.

No injection site or system reaction for first 7 days post vaccine, on day 8 patient noted raised red rash/hive at injection site. No pain, no pruritis, no blisters or streaking. No other symptoms at all. Advised to use ice pack, OTC Tylenol or ibuprofen for pain and swelling, and cetirizine for itching. If site becomes more painful, very itchy, blisters develop, or any systemic symptoms such as trouble breathing, etc, seek immediate medical attention.

Hard lump, redness, itching, swelling, and pain at injection sight. Symptoms seemed normal for first few days as they are common. But it is now 5 days since the vaccination and the redness, itching, and lump are not resolving.

It started with bad arm pain at the site of injection. I went to bed around 12:30 but I couldn?t fall asleep. By 3am I figured the only reason I wasn?t sleep was from the vaccine. I then started to feel extremely nauseous and had chills. At 3:30 am I took my temperature and it was 100.5. I laid in bed until 5am with no sleep still and took my temperature again and it was 103.5. I went back into my bed and fell asleep around 5:30 am. The next day I felt as though I had been hit by a truck. I felt extremely groggy and achy and my temperature when I woke up was around 100. Overall, I felt awful.

Throat tightness, chest pains, tingling all over body, headaches Immediate reaction treated with Benadryl and PEPCID injection Hospitalisation with a subsequent PEPCID injections, 10mg Claratin, 125mg Prednisone (1500), 2nd Prednisone at 2000, potassium chloride 20m at 2242. Following morning followed up with PEPCID at 0545, Claritin 10mg 0545, zenax when I returned the hospital after being discharged. Went back as I was experiencing chest pains and very flushed and red in the face.

Approx 15 min after vaccination, patient C/O tightness in chest and SOB. Rash began to appear on face and back. POD vaccination site was held in local hospital. Patient was taken to ER. She was checked in by triage. Patient left ER without being seen by MD. Was followed up by Nurse. Is doing well as seems to have recovered.

Intense itching at injection site, large swelling, heat at injection sight, pain, breathing difficulty, tight throat

The day of vaccination my top lip started to swell and small rashes formed on my arms, neck and butt around 8:30 pm. The following day my bottom lip started to swell and so did my left cheek at 8:30pm. Around 11:30 pm I started to develop a fever, feel soreness and weakness throughout my whole body, and a migraine. In the morning at around 8 am I noticed that I broke out in a severe rash all over my body. It has continues to spread all over.

Soreness in arm that lasted around 6 days after vaccination. Fever the night after vaccination. 3 days after vaccination, a red rash appeared around the vaccination site as well as along the side of my chest. I had a lot of pain in my armpit as well. The pain and rash went away for 3 days, and then returned with a red swollen area on my left arm at the vaccination site.

Patient called stating she had a "bad rash" present "all over her body" for one week post vaccination. Scheduled patient with appointment with provider for further evaluation.

- Joint pain - headache -fatigue -nausea -vomiting -diarrhea -syncope -chills/shaking -sweating -injection site soreness

During the 15 minute observation period post vaccination, developed itching, small red bumps on both upper arms, slight redness across chest. Vital Signs stable, no shortness of breath. Advised to obtain an antihistamine such as Benadryl and take according to package directions. Was with grandfather who was driving. Kept at observation site and additional 15 minutes. Advised to call primary physician or go to emergency room is symptoms do not resolve or become worse.

Severe all over body rash, including arms, back, legs, and face. Took benadryl for two weeks, did not subside. Received triamcinolone acetonide injection and medrol pak perscription on 3/22/21. Symptoms have subsided but there is hyperpigmentation on skin where rashes were.

Syncopal Episode - VSS Fell from chair, hit L forehead. Ice applied, large swollen red knot noted. Medic called to ER for evaluation.

I got my shot, sat down, and was reading through the information that the clinic provided me. A few minutes after, I noticed my right arm was getting warm, but I thought I was being paranoid (placebo effect). I began to feel nauseated. About another 5 minutes after feeling warm and nauseous, my ears began to ring and my vision went white. I was cold but sweating. I have passed out a handful of times before for various reasons, so I knew what was happening. I tried to signal to a volunteer from my seat that I needed help, but I ended up laying on the ground by my chair before someone came over. Once I laid down, the attention of the volunteers and nurses was on me. Three came over to assist me. They held my legs up, kept a hand on my wrist for a pulse, and fetched water. I talked myself through the dizziness and ringing in my ears. They put a cold rag on my forehead and had smelling salts on hand. Once I began to feel better, they helped me slowly sit up, and eventually helped me stand up and get into a chair. They kept a close eye on me for the next 30 minutes to make sure I didn?t have any other reactions.

Red round rump below the deltoid muscle; Red round rump below the deltoid muscle; Injection was wrongly administered at the wrong muscle site; A spontaneous report was received from a nurse, concerning a 19-year-old female patient, who received Moderna's COVID-19 vaccine (mRNA-1273) and developed red round lump below the deltoid muscle/ vaccination site mass and vaccination site erythema, injection was wrongly administered at the wrong muscle site/ incorrect route of product administration. The patient's medical history was not reported. No relevant concomitant medications were reported. On 06 Jan 2021, prior to onset of event, the patient received their first of two planned doses of mRNA-1273 (Lot number: 025L20A) on right arm for prophylaxis of COVID-19 infection. On an unknown date, after vaccination, the patient developed red round lump below the deltoid muscle and the patient's mother believed that the injection was wrongly administered at the wrong muscle site. Action taken with mRNA-1273 in response to the event was unknown. The outcome of the event red round lump below the deltoid muscle was unknown. The outcome of event injection was wrongly administered at the wrong muscle site was considered resolved on 06 Jan 2021.; Reporter's Comments: This report refers to a case of Product administration error for mRNA-1273, lot # 025L20A with associated AEs. Based on the current available information and temporal association between the use of the product and the start date of the events, a causal relationship cannot be excluded.

Pt with history of allergies, resting in recovery area after vaccine at 1351, patient reported feeling mild itchy throat, gave 25 mg benedryl PO at 1415, RR 22, HR 80, patient reported various allergies and that she had an epipen at home that she had never used, patient also had eczema and face appeared red and blotchy, patient stated that her face is often red. Lung sounds clear bilaterally. Patient began to appear more red on face and throat, started with bronchospasms and coughing, swollen tongue, increased tightness, administered Epinephrine 0.3 mg left thigh (lot 026C20AA, Expiration 02/2022). Patient immediately experienced decrease in difficulty of breathing, redness and rash decreased on face and neck, EMS called, Patient transported to hospital via ambulance, mother to follow in car.

Moderna COVID-19 Vaccine EUA Headache and tiredness for 4 days and counting Rash appeared on the 4th day

Site: Pain at Injection Site-Mild, Systemic: Fainting / Unresponsive-Medium, Systemic: Headache-Mild, Additional Details: Patient stated that she last ate around 10 am and had no extensive activity prior to vaccine at 6 pm. Per vaccinating pharmacist, patient had been fasting and fainted 5 minutes following vaccine administration.

STARTED AROUND 12:30PM ON 4/5/21: REDNESS, SORE, NODULE, BLISTERS AT INJECTION SITE, RED ARM. TOOK TYLENOL AND IBUPROFEN

About 15 minutes after receiving the vaccine, teammate went to stand up and got dizzy. Pt advised to sit back down and wait a bit. RN out to assess pt and again pt was ok sitting but upon standing noted wobbly and asked to sit back down. She did complain of dizziness at this time with some slight shortness of breath. BP noted in 120's with several checks, diastolic BP noted in 70's with heart rate noted 90-110 which per pt elevated HR normal for pt. At this time she did state that she was having some cardiac workup completed and just had on that was removed on 23rd March. At this time pt was brought back to treatment floor for further evaluation with ambulation assistance by RN. Pt was placed on oxygen 2 liters. BP's remained normal in 120's and HR as well between 90-100. Each time pt felt ok and went to stand she became dizzy and noted wobbly thus asked to sit but BP's remained normal. Pt states she ate some cucumbers and another fruit prior to coming. Pt at 1230 up to restroom with noted vomiting. After vomiting pt stated she was feeling much better. BP noted 124/84 temp 98.1 with c/o of n/v but no other symptoms while in restroom. Left arm noted some patchy redness after pt was itching the arm. At this time pt was given Benadryl 25 mg 2 tablets PO. Continued to monitor pt for about 30 minutes with redness subsiding on her arm, while all vitals remained stable. At this time pt was eager to leave and head home. at 1300 BP noted at 119/73 HR at 100. All symptoms subsided and while pt was standing no wobbling noted. Dr. was phoned and notified of the event as he is the medical director. Pt was given RN number to call when she arrived home.

Strong dissociation right afterwards Migraine, fever, extreme injection site pain, and weak limbs/lightheadedness the morning after

No signs of anything other than a large, warm, red patch surrounding the site of injection. It started as a solid red patch 7 days after the injection and is now just a ring on day 8.

Moderna COVID-19 Vaccine EUA. Sever nausea, dizziness, light headed, pain at the site. Staying stationary helps.

"Alerted to reaction by vaccine administrative staff on 03/09/2021 after they had received a message from the patient's mother. Note that patient received dose on 01/01/2021 and had already cancelled her 2nd-dose appt. Per phone conversation with the patient: Approx 3h after receiving 1st dose Moderna COVID-19 vaccine developed facial swelling (more on the left side of her face). Next day, awoke with some difficulty breathing and hives. + difficulty swallowing. Self treated with ibuprofen and antihistamine without significant improvement. Saw PMD, who prescribed PO steroids. Sxs resolved approx 1.5 wks after onset. Did not require hospitalization. PMD rec'd that patient not receive 2nd dose."

All occurred after 2nd vaccine alone: Swollen lips - approximately 4 hours after vaccine; started as a red line outlining lips followed by swelling and went away after an hour. Lips became swollen in the same manner again two days after receiving the vaccine and went away an hour later. Raised rash at injection site - approximately 8 hours after receiving vaccine - Rash grew in diameter for two days before shrinking. Malaise related events - began approximately 8 hours after vaccination, was at it worst the day following vaccination and began lessening two days after vaccination, full recovery about 5 days after vaccination - fever (reaching 102), muscle aches, migraine, extreme fatigue on day 2, brain fog

Rash present starting on day 12 after vaccination. Rash present on all extremities and neck. Not present on back or belly. Itchy.

At 1324 patient reported rash on left forearm. It was red, not raised, and itching. She had the injection in the left deltoid. Pt was given Benadryl 25 mg, felt comfortable driving afterward. No change in rash.

Rash covering upper arm 24 hours after I initial vaccine, issues with severe indigestion and bloating (countered with probiotics within a few days), then candida infection on genitals, under breasts, and armpits (treated with over-the-counter miconazole nitrate suppositories and topical cream, and prescribed fluconazole.) No past history of any of these symptoms nor conditions.

Red ring around injection site one week after injection. Itching at injection site and all over body, particularly upper left arm and face 5 days after vaccine.

Immediately after shot, pain and soreness in the arm. Next morning, 2 inches around injection site is painful to touch. Woke up with throbbing headache, hurts worse when I move. Constant throbbing headache and felt slightly nauseous. Has not even been 24 hours since vaccinated.

Red, intensely itchy rash on both hands (predominately around knuckles but some between fingers, tops of hands and on palms), both elbows (but most extreme on left elbow) and on soles of feet. The rash developed roughly 58 hours after vaccine. Used cortisone topically and took Benadryl orally. Symptoms subsided after 2 days.

Redness, swelling, heat, inflammation, and pain with movement or touch at the injection site. Nausea, vomiting, low fever (100.5), body aches and pains (random muscles having strong pains at different times), headache, all skin hurt to touch, similar to a sunburn. 11 days after and injection site is still swollen and appears to be bruised.

Patient reports after 1st moderna vaccine, 8 minutes later she experienced tremors and felt disoriented and her fingers turned blue. Went ot ed where she was diagnosed with panic attack. Symptoms ahd resolved and she was discharged home. She got her second vaccine on 4/8 and that night became very disoriented and acting odd (crawling around) and then became pale and was speaking incoherently. Was taken to ED where she had normal CBC, CMP, CT brain and CXR and then discharged home with dx of panic attack. Over the next day she felt much better and feels back to her normal self.

Received Maderna #1 at 17:15 hours. Developed a rash in her upper chest with itching - Lungs clear - No SOB - no stridor - Capillary refill intact. 140/82 - o2 Sat 99 - HR 75 Assessed by Dr. - Benadryl administered by physician - Ambulance transport at physician request to the ED

patient developed left upper arm reaction post covid vaccine. also had to use regular inhaler more often than normal (patient has seasonal allergies). arm developed red circle around the injection site (covid arm) give albuterol and solumedrol in the ED. discharged home

15 minutes post second dose of vaccine patient reported a rash on the left upper extremity surrounding the injection site. Patient was given 25 mg of Benadryl.

Vasovagal Syncope w/Subsequent Head Injury - Approximately 10 minutes after vaccination, the patient sustained sudden onset of lightheadedness and lost consciousness. Fell from seated position in her chair to floor. Sustained contusion/abrasion injuries to left periorbital region. Appropriate clinical actions undertaken and IVF started. Patient stabilized and transferred to ER via EMS.

Patient states pain at injection site beginning the day after injection. Pain has moved from the left arm to shoulder and then to the neck. Patient is also experiencing dizziness.

Patient has rash/hives on trunk, arms and legs. She stated she was lightheaded/dizzy, which lasted around 30 minutes. Her hands, arms, body had tremor and were shaky. She had blue/purple coloring of palms, which were cold and swollen. BP was 84/65 on my machine. HR was 110 bpm, minor lip swelling. Patient was given Benadryl oral 25 minutes post vaccine and EMS were called 35 minutes post vaccine due to concern.

Within 5 minutes after receiving vaccine, patient indicated that it felt like her whole body was on fire and she felt faint. She was already seated for observation following the dose. An Epi-Pen and ammonia inhalants were removed from the emergency kit for possible use. Pt stated she was not having difficulty breathing and no closing of her throat. The ammonia inhalant helped the patient focus and she stated that it made her feel better. Pharmacy staff called 911 and the EMS crew arrived in approximately 5 minutes. They evaluated the patient and transported her to the local emergency room. A couple of hours later the patient called and said she had been discharged and was doing well. While in the emergency room she broke out in hives and was given epinephrine and benadryl per her conversation. Later her father came to the pharmacy and confirmed the same. She will see her primary care doctor on Monday.

PATIENT RECEIVED HER 1ST MODERNA SHOT AT 1:15PM AT OUR PHARMACY. IT WAS GIVEN BY NURSE. AT 1:30P.M. PATIENT STARTED C/O ITCHING ON BILATERAL ARMS AND SLIGHT RASH ON BOTH LOWER PORTIONS OF HER ARMS. I ASSESSED FOR ANAPHYLAXIS SYMPTOMS, THE PATIENT HAD NONE. SHE HAD NO OTHER ADVERSE SYMPTOMS. I ADMINISTERED 50MG OF DIPHENHYDRAMINE IM TO LEFT ARM. THE PATIENT WAS ACCOMPANIED BY HER MOTHER. I REACCESSED PATIENT 15 MINUTES LATER AND SHE FELT A LITTLE BETTER. THE PATIENT LEFT AROUND 20 MINUTES AFTER THE INJECTION WITH NO FURTHER COMPLAINTS. SHE HAS AN EPI PEN AT HOME, HAS USED ONE BEFORE AND COUNSELED HER IF AT ANYTIME SHE WERE TO HAVE S/S/S OF ANAPHYLAXIS TO USE HER PEN AND CALL 911. I CALLED THE PATIENT ABOUT 20 MINUTES AFTER THEY LEFT TO CHECK ON HER, SHE SAID SHE WAS SLEEPY BUT SHE WAS OK.

48 hours after vaccine, patient began feeling very ill. She went to bed and did not feel like doing anything. She developed a rash on both arms, breasts, and left leg. On the same arm as the vaccine, she had a huge bruse with a welp around it, but the bruise and welp were on the under side of her arm. Patient went to walk in clinic then to ER. They gave her a steroid shot, and prescribed oral and topical benadryl and a steroid pack. As of 4/29, the patient was improving.

4/28 @ 4 pm day of vaccine, lower back achy & stabbing pain, like laying on nails. 1 hour later progressed to include middle back, by 6 pm entire back to base of neck in pain & stabbing pain would focus on 3-4 spots and move around. Fever 99.8 @7:23 pm, 100.5 @ 7:46. Bad chills all evening. Advil taken. 4/29 back pain improved when fever decreased but worsened as fever increased. Advil & later tylenol. Hot & then cold throughout day. By 1 pm, back pain just middle & lower back, by 5 pm just lower back, woke on 4/30 and back pain gone. Arm pain at injection site began 2 hours after vaccine, couldn't raise arm more than 2 ft from legs, pain better but still persists.

Waited my 12 minutes left, arm was sore but that was to be expected, some fatigue but then days afterwards, i also wear a facemask in p ublic except the gym but i also go when im usually the only one thats there or maybe a couple people there, then i had a nasty sore throat i couldnt really speak at my usual volume the week prior i had a runny nose but i was fine when i received the vaccine. it comes and it goes but i also realized i had little red bumps on my arm then they went away with no medicine in under 24 hrs which was also on my left arm, then i got fatigue, but the sore throat keeps coming and going i wasnt sick when i received my first dose i also had a headache later after the first dose and slept it off, i didnt go to the hospital because i didnt think it was bad enough for me to go. i had covid 19 last july

Patient arrived to receive 2nd dose of Moderna on 5/5/2021 at 10:18. five minutes into the 15 minute post COVID vaccine administration monitoring, patient started experiencing side effects: 10:23:Patient reported dizziness, lightheadedness, became pale and clamy, with nausea. Patient request water and was given 16 oz water bottle. Patient reports that she did not eat breakfast, has no hx of allergies, and has hx of anemia. Patient currently taking Iron and B complex supplements. BP 52/33 P 53 o2 Sat 91% on RA. Patient took a bite of a brownie. 1025: medical response team arrived. Patient assisted to laying position with feet elevated. 10:28: Pulse 64, Pulse ox 91% on RA 10:30: Verbal order received to assess blood glucose. BS 79, BP 88/58 P 62. Denies nausea 10:33 Verbal order received to give Glucose 4g Tab by mouth on-time. Patient consented. 10/35: Patient denies nausea, denies dizziness, skin is dry and skin color is normal for ethnicity. BP 92/61 P 81 o2 Sat 98% on RA 1036: reassessed patient. Patient continues to improve. monitor patient for 10 more minutes in flat position then assist patient to sitting position for an additional 15 minutes and continue to monitor. 10:40: BS 118 1041: 96/64 P 70 O2 sat 98% on RA 1046: Patient assisted to sitting position and reported light dizziness. BP 87/57 P 65, o2 sat 94% on RA. Patient drank 8 oz. of water. 1051: BP 86/58, P 70 o2 sat 93% on RA. Denies Dizziness 1052: reassessing patient. Patient denies dizziness, denies lightheadedness, denies nauseouosness, skin dry and skin color normal. Patient to be monitored for an additional 12 minutes. If continued progress patient is ok to go home. Patient has driver present at this time. Patient encouraged to change positions slowly, continue to hydrate, eat meals. If issues arise and continue patient encouraged to contact her PCP or visit closes urgent care or ER. Patient verbalized understanding.

Symptoms started off Wednesday night around 10pm with itchiness all over the body (didn?t think any of it). Thursday morning, the itchiness had ?spread? to my neck (main itch spot) and I saw a nurse practitioner to help out (prescribed benadryl and cortisone which didn?t work). Friday morning, the itchiness is still there and it?s getting annoying at this point. Saw another doctor who prescribed benadryl at night and Loratadine in the morning/afternoon, as well as colloidal oatmeal baths, new detergent and shampoo, dove unscented soap, and caladryl (did not help at all). Friday night, both arms and hands were feeling numb and tingly. Went to the ER saw a PA and he prescribed Benadryl 25mg, Pepcid 20mg, and steroids 60mg (Med....Prednisol ???) Kept up with the prescription on Saturday 25mg of Benadryl, 20mg of Pepcid and started of the 21pill package of steroids (6pills on the first day, each 4mg, and tapering off 1 pill per day for 6 days). The treatment didn?t work. The itchiness and discomfort is still very much apparent. Tuesday (05/04) afternoon I saw a primary care doctor, explained the symptoms and treatments. She believes it has something to do with the vaccine and described my symptoms to be similar to that of ?dermatographia?. I don?t have a history of allergies or allergic reactions and the only skin ?problem? I?ve ever had is acne. No pre-existing medical conditions that could ?trigger? this

I became very tired and had body aches two days after my second dose of the Moderna vaccine. After that, it had been about four days since I had received my second dose. I started to notice rashes appear all over my body. They were very itchy at times. So then I went to my local urgent care on May 4th, 2021 to see if they knew what was happening. They prescribed me Hydroxyzine for the itching and Prednisone for the swelling of the rashes. As of May 5th 2021, almost all of the rashes are gone thanks to the medication.

Exhaustion, muscle soreness, nausea, chills, and fever starting around 12 hours after vaccination administered and lasting for 12 hours. Vaccination site pain starting directly after administration and lasting 24+ hrs.

Chronic hives reactivated. One of the worst outbreaks since inception of hives. Edema in lips and eyelids. 72 hours past injection, hives ongoing, currently being treated with prednisone.

A large raised, red welt at the injection site around 6 hours after the shot. Lasted around 4-5 days. Very hot and itchy. No treatment. Left a slightly discolored patch on my arm

Patient was given shot and mother reported patient was not feeling well about 5 minutes after shot was given. When asked, patient had some dizziness, skin was a different tone of color (per mother), and stated that they were breathing rapidly. Beverage was given to patient and ED called.

An itchy and sometimes tingly swollen welt/rash at the location of the injection. Firm to touch and about an inch and a half in diameter. Additionally a swollen lymph node along the collarbone.

At approximately 3:29PM on 5/10/21, began to have adverse effects after receiving her Moderna dose #1. She temporarily lost vision in both eyes as well as her hearing. She became cold to the touch, red, and clammy. Patient reported feeling like she was feeling like she was blacking out. We had given her water, wiped her off with towels, and fanned her. 911 was called at this point and her blood pressure was checked which showed 122/91mmHg. Management came over to the pharmacy and was helping fan her as well. The fire dept and EMTs showed up soon after and took over. Patient?s hearing and vision were restored and started feeling back to normal. Patient then declined further medical intervention from EMT?s and went home.

Pt. c/o hives and swelling of entire right arm developing later the same day of her 2nd Moderna vaccine. By 5/5, she had wheezing, coughing, itchy throat and eyes, and redness of face. As of 5/7, she is still wheezing and has a bruise at the injection site. Was seen at an urgent care center and advised to take Benadryl. It is minimally helpful and wears off quickly and the symptoms return. Pt. is currently uninsured and was given information for low-cost clinics. She was advised to have wheezing evaluated.

Early Sunday 5/9, she was sore all over her body, achy, injection site was hot to the touch with red ring. Symptoms started to subside on Monday 5/10/21

At 10:00 pm I started with a stomach ache (12 hrs after my vaccine). By 10:05 pm I was experiencing nausea, vomiting and diarrhea. Within 10 minutes, my face had swollen up, my eyes were swollen and red, and my hands, feet and chest had hives. Paramedics were called and arrived 5 minutes later. At this time I was barely able to walk, was still vomiting and having diarrhea, and was shaking. They put me in the ambulance and took me to a 24hr emergency clinic. In the ambulance, I was given saline, epinephrine, Benadryl and nausea meds. I stayed under observation until 3 am. I was released to go home at that time. Since then I have only experienced weakness and nausea.

100.0-100.5°f fever lasting 24 hours; Red rash around injection site about size of index finger down front of arm and 2 index fingers around arm. Rash is very hot to the touch and very sensitive to touch. It happened with the first vaccination as well.

About 6-7 hours after receiving the vaccine, I started to develop hives around my body, including on feet, stomach, ears, face, back, and legs. I had fever and chills, muscle aches, joint pain, headaches, and nausea later that night as well. The next morning, my left eye was swollen, left side of tongue completely swollen, lips swollen, and left side of throat closed (anaphylaxis). I took prednisone, Benadryl, and Tylenol to relieve the symptoms, and the swelling around my left eye, tongue, throat, and lips went away. Throughout the day, more hives have developed around my body and face, including my right eye. I am continuing to take Benadryl and Tylenol and finish doses of prednisone. The fever and chills, muscles aches, joint pains, hives, headache, and nausea are still present.

-FIRST HOUR, ARM GREW EXTREMELY SORE - LATER THAT NIGHT, HEADACHE, BODY SORENESS AND BACK AND FORTH CHILLS OR SWEATSING (SOAKED SHEETS SLEEPING). RELIEF ON 5/14 EXCEPT FOR RED RASH AROUND INJECTION SITE.

May 14th, 2021 at approximately 23:00 I started getting a rash/raised lesion on my left arm at the injection site, which wasn't painful-just itchy. It went away after a while and is fine now.

About 1 hour after patient received vaccine from here, she called and said her throat and tongue started to itch and swell. She was experiencing itching all over her body as well. She said she took some benadryl and the symptoms got better, but wanted to report the event. At the time that she called and notified me she was doing fine.

pt. received covid immunization on 5/8/21. On 5/16/21 patient has reddened circle, slightly warm to touch. Noticed about 4pm. Advised patient this is a common reaction, to keep track of it, if worsens to be seen, take ibuprofen. Patient felt fine.

I had a red, itchy rash beneath the vaccination site that appeared 7 days after my vaccination. It went away after about two days, then came back the very next day. I drew a circle around it. Overnight, it got larger but less red. Throughout the course of the day, it was itchy and warm to the touch. Over the next two days, it gradually got smaller and lost the red color and warmth. About two days later, the redness came back slightly and was barely warm. It was no longer itchy. It?s been three days and I have not had any more issues.

Itchy, burning, and swollen. If I itched it it would get worse so I avoid touching. When showered I would let my arm stay under the warm/hot water. It would look better for a day or two and it would get worse. It?s been a few days that it healed.

States that she immediately felt dizzy and then passed out. The person sitting next to her said she suddenly fell to her left and hit her head on the concrete. Has a hematoma to the left side of forehead. Following fall threw up three times. Blood pressure initially low but has now returned to norma. Is now alert and oriented and refusing transport to the hospital. Per medical direction she can refuse and is having a friend pick her up and take her home.

Patient states 8-10 hours after receiving vaccination she began developing rash on arms which progressed to most of body. Pt currently monitoring for worsening of rash or anaphylactic event.

About 2 hours after the vaccine being administered, patient experienced hives, ger face became red and swollen, and she was itching all over her body. She went to the ER , her heart was racing, they gave her benadryl injectable. Soon after, she started to feel tired and sleepy, and during the next day she was feeling still tired. The day after, she is feeling much better, still experience a little bit of itchiness at forehead and arms, seldonly.

First dose of Moderna: received vaccine April 23,2021 woke up April 24th my lips were swollen, blue and itchy. I had hives all over. I called my aunt that?s a doctor she told me to take Benadryl which helped. The swelling on my lips lasted until April 27th, and only one side swelled up . 2nd dose of Moderna: I had a fever, chills, swollen lips, itchy tongue, my lips also turned blue. This time the opposite sides of my lip swelled up and it?s extremely painful when rubbed against. I can breathe so I?m not worried. Allergy medicine seems to help with the swelling and itchiness. My nose is stuffy and i have a lot of abdominal cramping.

Patient had a delayed reaction she was in the parking lot after 30-40 minutes from receiving the moderna shot and she started having hives, threw up, and had massive headache. She had epi-pen injected into her leg and ems was called and came to pick her up after about 20 minutes from epi-pen injection.

I got my second dose of the moderna vaccine at 10 am on 05/26/2021 and maybe 4 hours later my body stared to break out in a rash all over my chest back and both arms. The rash is itchy and has not gone away or gotten better a day later and it still seems to be multiplying. I felt difficulty breathing but I cannot be sure cause I was nervous I may have been scared after seeing the rash on my body.

Client reported hives on right arm 10 hours after receiving vaccine. Per client took Benadryl and went to sleep. Next morning hives had disappeared. Per client no current medical history, no medications and no known allergies.

Mother alerted pharmacists before administration that their daughter experiences syncope with needles. The mother sat by her side as I administered the vaccine. The patient said that the vaccine hurt and sat their for about 5 minutes . At that time the patient said she did not feel well and laid on the ground. It is unclear if patient passed out for a few seconds. I spoke with patient to make sure she was conscious and then monitored her to look for signs of anaphylaxis. Her symptoms appeared to by related to anxiety rather that anaphylaxis. The pharmacy did call 911, but the patient and her family turned down the emergency service. I sat with patient for about 45 minutes to monitor her condition. When she improved they decided to leave that pharmacy. I did suggest they be looked at by a physician. We also called and check on patients well being and she seems to be doing fine.

Redness, itching and mild swelling developed around the injection site one week after first vaccination.

10 days later, new rash on lower extremities, nodular. Did not recur with second dose, unclear if related.

Patient called with concern that her vaccine was administered too low on her arm, with complaints of redness, swelling and feeling of numbness/tingling down her arm. Later that night she lost all sensation and strength in her arm. Patient sent photos with site of administration the next day. Site was red and swollen and clearly administered too distally and medial. Patient was advised to reach out to her health care provider to be assessed. Patient stated her health care provider agreed this was incorrect placement but no further treatment or intervention was provided. patient stated it took 3 days to regain sensation and strength . Site took a couple weeks to reduce in redness and swelling. Site is still darker in color, decribed as a "birth mark appearance" with a red spot in the middle at the injection site.

My arm broke out in red rash and hives. Over the next couple days it spread all the way to my ankles.

Pt complained of feeling dizzy and bilateral arm tingling/pain 15 minutes after receiving vaccine. Pt was observed and periodically checked on for approximately 1 hour, she then stated that her arms and legs were itching. I gave her 50mg of liquid diphenhydramine, over a period of 2 hours she was observed and checked on, around 5 pm she stated the itching was gone and she felt fine. I gave her a bottle of diphenhydramine liquid and directions for use. I followed up with the patient the following day and she said she continued to feel fine.

Fast-paced heavy breathing, trouble swallowing, numbness tingling in arms and legs, sharp pain in left and right forearms, headache, nausea, vomiting, weakened legs left and right, itching in arms and legs both left and right, hives and bumps in right vaccinated arm, anad hives developed around lips and mouth area.

At 2 PM on 6/6/2021 I received the second shot of the Moderna Covid-19 vaccine. At approximately 1 AM early morning of 6/7/2021 patient experienced Chest pain and shortness of breath. At approximately 4 AM I woke up and was itchy in several places on my body and saw rashes which were red and raised located on back of neck, both hands, both arms, her right hip behind both ears, left knee, right thigh and left shoulder, right upper mid-chest area to right side. After that took an immediate shower and then left to go to the Emergency Room and arrived approximately at 5:30 AM. ER Doctors gave Prednisone and Benadryl at 6:10 AM. After a few hours rashes appeared to start to leave.

She was seen in emergency room and given injected steroids to treat reaction.

Patient was seen at hospital on 04JUN2021 for a Purpuric Rash at and was refereed to Medical Center on 04JUN2021 time 1704, upon arrival to the patients labs were collected. Lab results confirmed Thrombotic Thrombocytopenic Purpura due to platelet count of four. Patient has been hospitalized since that date receiving treatment.

Patient reports injection site pain and redness at injection site. Also complains of nausea and vomiting 1 day after receiving 2nd dose of vaccine. Asymptomatic with 1st dose. Symptoms resolved on day 2 after receiving vaccine.

Patient reported noticing a mild red rash on her ankles the night of her second Moderna shot (7/12/21), and noted that is started worsening the following night (7/13/21) and was spreading up the shins. Continued to spread and develop red welts that were painful to the touch over the next days. Reported to the pharmacy the evening of 7/15/21. Counseled patient to apply hydrocortisone cream to the affected areas, to take a dose of diphenydramine, and to contact her primary care physician for further evaluation. Later that evening, she was prescribed triamcinolone ointment to treat a diagnosis of hives.

The first time I got my vaccine on April 27, 2021 I have gotten a bad rash on my right arm on April 29, 2021 than when I gotten my second vaccine on June 06, 2021. Two days later on June 08, 2021 the rash was on my left arm as well. On June 19, 2021 the rash went away on my left arm and gotten worst on my right arm. The rash is itchy and time to time I get sharp pain in my arm. I went to the doctor for the rash and talk about it. The doctor told me that I was having a allergic reaction to the vaccine that I have got. I have gotten the Moderna vaccine on my right arm both times.

The first time I took the vaccine I gotten a rash on my right arm than when I gotten my second vaccine the rash gotten worse on my right arm and I gotten a rash on my left arm but it was not that bad. Couple weeks later the rash went away on my left arm. The next week the rash on my right arm has gotten worst the rash goes from my wrist to my elbow. The rash on my right arm itches and time to time I get sharp pain in my arm. I went to the doctor and the doctor said I might be having a allergic reaction to the vaccine.

27Dec2020 - patient developed some swelling (went about 1 inch out from injection site) and discomfort at the injection site - had taken Advil and was using a cool compress. 29Dec2020 - patient had to leave work early since she was not feeling well - very fatigued and a little dizzy (able to eat/drink) - denied any COVID illness s/s (no cough, fever, loss of taste/smell, etc). She ended up going to the local ED - since she was still very fatigued and was feeling like she was going to pass out - blood work and U/S done - this is where she found out she had been pregnant but had lost the pregnancy (no heart beat). Per her report - all other blood work was unremarkable. No other medications given to her during this visit and was just told that she would be 'passing the products of conception within this next week'.

redness, heat, and swelling at the injection site. three and a half inches by 5 inches and expanding. went to the urgent care and the provider prescribed antibiotics, taking the antibiotics if the symptoms worsen in the next 24 hours.

Received shot around 4pm. First 10 minutes, then arm started throbbing. started getting dizzy and fatigue. Had weakness and abdominal pain, sweating profusely. Couldn't cool down. Felt winded, severe abdominal pain 10-15 minutes. Still having pain at the injection site. started having chills and then started to fell better.

One week after my shot my arm had became itchy and red. Now it is not itchy, but it is red and the swelling spot has grown from a small mosquito bite size to the size of my palm. Very warm to the touch and my right arm is sore.

Shot arm became itchy and small red bump formed. Over the past 24 hours, the bump grew and is still red and warm to the touch. Soreness throughout right arm where I got the shot.

Pain at injection site, chills, cough, sore throat, runny nose, congestion, headache, fatigue, nausea, delirium, chest pain for 4 days now.

An hour after I became super itchy -8:00am 12:30- chills began 1:30 body aches 6- low grade fever, fatigue Next day- sore throat, headache, body aches, chills, fever of 104.6 I was shaking so bad from being cold Then I had night sweats

Patient vaccinated in left arm on 12/31/20 on 1/8/21 noted spot of redness, swelling and pain. Not sure if this is a cellultis

Pt complaining of dizziness and SOB and had mild hives at injection site and posterior shoulder. Pt was given Benadryl IM and transported to hospital by paramedic ambulance.

Developed migraine and loss of taste 12/30/20 (currently being tested for COVID-19). Developed right (side of injection) axillary pain 1/2/21 which lasted several days. Hives developed on 1/7/21 - ordered Benadryl without relief. Went to ER 1/9/2021 for worsening hives - Pepcid and prednisone ordered.

Patient received Moderna Covid19 vaccine at 1500. Was instructed to wait in vehicle; five minutes passed and she proceeded to exit her vehicle to report symptoms. As she approached assistant, she feel forward landing directly on her chin. She loss consciousness for approx. 10 seconds. Was assisted to left side and was noted to have a 1 in laceration to the bottom of her chin. "911" called for assistance. She regain conscientiousness shortly after and became alert and emotional. She was assisted to her knees position as she became agitated and anxious. Vital taken: 105/74 P: 72 R: 32 at 1507 At this point 1508 due to agitation Benadryl 12.5mg administered to L deltoid. Patient began to complain about inability to mover her hands and feel her fingers. Reassurance given that she was able to move her extremities as she was able to squeeze the hands of the nurse as commanded. EMT arrived, report given to paramedic and care released to paramedic and transported to ED with family following. As a note: family reports that she has been newly diagnoses with anxiety.

Fever (103-104 oF) and 4"x1" red, swelling area around site of injection. Pt states she received an IV.

Moderna COVID-19 Vaccine EUA. Large bump (1/2 inch raised from the rest of my arm) on injection site appeared roughly a week after vaccine. Roughly 2.5 inches in diameter and firm to the touch. Mildly itchy and red with a few small bumps.

On 1/18/2021, the site of the vaccine had a small lump and was itching a lot. Then at night, the rest of the body (arms, legs, thighs) were itching a lot so I took Zyrtec.

Felt chest warmness/pressure from minutes within vaccine. Brought immediately to medical area. Felt cold, looks comfortable. on phone. no immediate sign of distress. given orange juice and observed with father present. Patient released at 1:40 pm to her father, feeling and looking well.

7 to 8 days after shot I have been getting an allergic reaction in the left arm (where I received the shot) to the left of the injection site and slightly below. It comes and goes but will flare up and become itchy at random times. There is small swelling in the itchy area but not a huge bump on arm, only slight risen and very red, work/ hot feeling in the area. Currently feeling this now 1/22/21 as I fill this form out but it is going away, again.

~15 minutes after injection (while waiting in observation area) patient witness another patient have slight nausea and mild light headedness (which quickly resolved). About 5 minutes after, the patient began feeling nauseated and light headed. I was called to check on here. She was pale and and leaning forward over trashcan. After about 2 minutes of questioning her and getting baseline vital signs, she started to slightly improve. We assisted moving her to an exam room. We had her lay near supine (as moving brought on new wave of nausea and lighteheadedness). She was closely montiroed. About 10 minutes later we began sitting her up, but after only a few symptoms began to return. Laid her back into supine. finally after anothr 30 minutes we were able to fully have her sit up and stand with no residual symptoms other than fatigue. Through out event normal VS HR, BP, O2 (other than slight tachypnea initially). Normal physical exam (other early noted pallor and reticular rash on extremities, which all resolved). Advised patient that on next dose to ask to be given in a location where she can be laid supine and observed for at least 30 minutes after injection.

Received 2nd dose of Moderna Covid-19 vaccine on Monday 1/25/21. Patient experience fever, achiness, dizziness, fatigue, headache, low grade fever over the course of the next day. Patient presented in the ED today 1/27/21 after a feeling of a rash, welts, and a feeling like burning.

The first dose I experienced no symptoms. Only had pain in arm. The second dose I experienced chills, fever and allergic reaction at site of shot.

Fever 100, sore throat, body aches, warmth, tenderness and redness at injection site, approximately 3", increased in size overnight since onset. Recent hospitalization for CD. Tested negative at hospital discharged 1/19.

Approximately 15 minutes after receiving vaccine patient had reddened area around injection site that extended down her upper arm to mid humeral area and extended up her arm over shoulder and across her chest to across midline in the front and upper back on her back. Area was warm to touch. Patient denied shortness of breath or any other symptoms. Patient had bled with injection. Area was marked with a pen and patient continued to be monitored. NP views area at 30 minutes post injection. Slight receding noted of redness in arm area. patient continues to deny any other symptoms. Patient instructed to return home and take Benadryl by NP. 1/15/21 patient contacted again by NP . Reports redness receded and was gone the night of injection, no other symptoms noted.

I have had flu-like symptoms (exhaustion, shortness of breath, body aches) every night since receiving the vaccine (1/24), which was a week and a half ago. Tonight, I now have extremely flushed cheeks, and they are warm to the touch. It is like a sunburn, but I have not been outside in the sun the past few days.

My throat started to close. I was itchy and had a difficult time breathing until I was given an epi pen.

Immediately after receiving the vaccine, I had pain at the injection site and swelling that has persisted more than 48 hours later. I experienced chills 24 hours after receiving the vaccine. Then around 8pm on 2/3/2021, I started feeling very nauseous and had a fever of 100.1 F. Now on 2/4/2021, I don't have a fever anymore and I feel fine, but I still have pain and swelling at the injection site.

Besides soreness and pain in the arm of my injection site, I had aches and pains in my abdomen, several anxiety attacks throughout the night due to pain, stomach aches, and nausea. I had two sets of hiccups that gave me a headache and caused severe pain in my chest and spine.

Covid arm (about a week after first dose, developed redness, itching, and swelling at the injection site)

On the night I received the COVID-19 vaccine, I had a fever that went up to 103 degrees Fahrenheit. In the morning 24 hours after I had the second dose of the COVID-19 vaccine, I felt a little better from the night's fever and ate instant ramen, oranges, acetaminophen, and a vitamin C tablet. This was at about 8:45 AM. A few minutes or a minute after I swallowed the acetaminophen and vitamin C, I got a severe allergic reaction that led to me going to the Emergency Room. I have eaten these things before and am not allergic to acetaminophen, vitamin C, or anything in the instant ramen. The reaction was first felt by a shortness of breath and I could barely breath. Then, hives started to form on my face and my lips and face became extremely swollen. I then ended up throwing up after my mom gave be a Benadryl pill and I drank another Benadryl after. My face had become less swollen but it was still difficult to breath and hives started to form all over my body so we decided to rush to the Emergency Room. We got to the Emergency Room at about 9:30 AM and the hives on my body were worse. They were able to calm down the reaction by giving me more Benadryl through an IV and steroids. They knew I had the second dose of the vaccine so they did not give me anything that would suppress my immune system. Timewise, it could be the acetaminophen, but I have taken acetaminophen in the past many times before without a reaction.

A large circular, red, itchy lesion appeared at the vaccination site. It was swollen and hard to the touch. It grew overnight and into the next day reaching a diameter about 4 inches. It healed and disappeared after 3 days. Itchy hives on the buttocks, back, and stomach occurred at the same time, appearing each night for 6 days. Benadryl was used on nights 3 and 4, and hives persisted.

PER PATIENT: "I BROKE OUT IN RASH ROUGHLY 6 HOURS AFTER VACCINATION ON MY NECK, HANDS, ARMS, CHEST, STOMACH. TREATED WITH AN OTC ANTIHISTAMINE AND LOTION. RASH LASTED ~48 HOURS AND ITCHINESS ALL OVER BODY LASTED X 1 WEEK."

Patient describes having a very red face with associated hotness. Some relief after oral Benadryl but redness remains after three days. Patient has been using cold compresses. No other reported symptoms.

Swelling of arm week after vaccine. Red, swollen lump on left arm. As well as swollen lymph ones under left armpit. The day after vaccine, my arm had the normal inflammation related to the vaccine. It went away after 4 days. A few days after my arm started to become itchy. It has been itchy since. On 2/11, my arm swelled up again and became red around the injection site and hard and warm to touch.

Moderna COVID-19 Vaccine I had a rash show up 8 days after my vaccine right around the area of the site of injection. It continued to grow over the weekend and is now covering a large part of my arm. I looked it up and it matched the pictures of ?COVID arm?

I have "covid arm" but it's in my neck/upper back. It is sore, swollen, and tender, with a raised bump and discolored skin.

Unsure exactly when started but definitely started at least 24h after vaccine. Deltoid was swollen and hard after vaccine which I think it normal, but I had redness sort of under it/toward the front of my arm. Today is now 2 days post vaccine and it has become itchy and painful. I looked it up and saw others are reporting similar things HOWEVER, mine is at least about 4-5 inches away from injection site. I think it may be a mild allergic reaction. I haven?t treated it yet because I don?t know what to do, and I?m hoping it won?t worsen.

Local Redness of Swelling 50 to 120 mm, but not below the elbow. Itching but no other symptoms reported. 50mgs diphenhydramine given IM Right Arm. Observation started for 60 mins. All symptoms subsided.

Began with headache at 3pm, progressively got more sore in joints until 5pm when my whole body was sore and it was difficult to move. Around 7pm i felt dizzy and chilly, at 1am i threw up and had very bad chills and was still sore, at 7am the next morning i realized my vaccine site was swollen and bruised and felt like a large lump, the skin was rough and very hot to the touch. over the next day i had rolling headaches and nausea and it was difficult to move without pain in my extremities, this started to lessen over the next days but my arm is still swollen, hot, lumped, and painful.

After first dose, got hives around injection site on left arm about 12 days later. Went away over night, but reappeared briefly a couple days later. After second dose, got same hives around injection site a couple days after vaccination. Comes and goes. On 2/22 got hives on left and right arms, back, and chest.

My entire face was covered with hives. Within 2 hours of the vaccine I developed hives to the bilateral aspect of my face, forehead, cheeks and eyelids. My hives were itchy and red and I have never experienced this before. I did not consume any new food or use any new products. I took Benadryl for the itchiness. I had a telemedicine visit and I also had a Covid-19 test which was negative. I called health and also reported this adverse reaction. Moreover, I had no reaction to my second dose which was on 01-28-2021.

On 2/12/2021, had post-injection symptoms of joint pain, cold sweats and dizziness On 2/19/2021, she noticed injection site was red and raised then became "big, red and swollen" with 3 "bumps" noted. Was seen at Health Center on 2/20/21 and was diagnosed with cellulitis, prescribed Keflex and Claritin. Was seen by this provider on 2/24/21 for follow up and she states that initial red area surrounding the injection site is improved, however, a large red ring is present. Also C/O an additional irregularly shaped red area distal to initial red area surrounding injection site. No infection noted. Instructed to continue Claritin and add Pepcid 20mg daily to help alleviate the histamine reaction occuring. Denies shortness of breath, sore throat, facial/lip/tongue swelling.

Severe nasua and vomiting, petechiae on face and neck, bone pain in left arm (arm that vaccine was administered), fever, chills

I developed a very large, swollen lymph node under my left axillary. I have never before had a swollen lymph node in either axillary regions of my arm. It was very painful, I could barely put my arm down because of the pressure of it pressing against the side of my body at rest. It has gotten better, there isn?t as much pain but it is still uncomfortable. The lymph node was/is palpable, and hurt upon touch and rest. The pain was an 7/10, but has now gone down to a 2/10. My arm is also still sore from the injection site, but it is not bad.

1st dose: Pain and soreness at the injection site lasting 5 days post-injection. Moderate headache 6 hours post-injection acetaminophen was taken 8 hours post-injection. 2nd dose: Pain and soreness at injection site. Moderate headache, fatigue. 6 hours post-injection 18 hours post-injection very severe headache/migraine lasting 12 hours fatigue lasting 24-72 hours post-injection mild to moderate muscle pain 24-72 hours post-injection mild to moderate joint pain/aches 48-72 hours post-injection mild nausea 24-72 hours post-injection very mild acetaminophen was taken 24 hours post-injection rash at injection site developed 72 hours post-injection at the injection site measuring 3 inches in length and 2 inches in width with jagged edges. Is red and hot to the touch and the injection site is sore and firm very mild itch.

I have felt injection site pain since about 10:30pm on March 1st, 2021. At about 12:00pm on March 2nd, 2021 I started to feel tired and developed a mild headache. I felt chills at about 3:00pm on March 2nd, 2021.

Extreme fatigue, unable to go about my day, constantly falling asleep. Started at 5am on 3/5, arm also sore at injection site, no skin reaction, started about an hour after the injection

Around 2:30-3pm I began feeling tired. I went to lay down and rest and started to get chills and a bad headache along with hot and cold episodes. I started to sweat but had no fever at the time. This continued all night. I had no appetite after receiving my vaccine. Around 6:30 I started to get a fever of 100.3 and continued chills with headaches. I took Tylenol when needed. My whole body was sore throughout the day and got worse are the night progressed. My fever was in and out but around 9pm I had the highest temp of 101.4. I didn't get good sleep due to feeling unwell all night. I belive I stared feeling more myself sometime around 9:34am. But was still feeling a bit warm. But felt way better as the day progressed. My left arm where I received my shot is still very sore. And I still don't have an appetite.

Hives on the torso and upper aspect of the limbs. It didn't happen until 3 weeks after the second dose, so it could be unrelated, but it is the only thing that has changed in the patient routine and environment.

Patient received a COVID-19 vaccine on 06MAR21 @ 1030. Approximately 2 hours later, she began developing a reddened area on her arm at the injection site which was hot to touch and became itchy. She reached out to our onsite nurse on 08MAR21. She iced the area and took Benadryl. She was told to follow-up with our onsite nurse if her condition didn't improve. On 10MAR21, she reported she has been taking Benadryl HS and the area is "almost completely cleared up."

Confusion, my ear got very swollen - it had hives and bumps on it and was itchy. I saw my doctor the next day. I got two antibiotics shot. I also started on antibiotics and this helped with the symptoms. It took about a week and a half for symptoms to resolve.

No rash on injection site. However rash on both legs and one arm had rash on elbow. The rash became dark purple and very large. I was taking Benedryl, Hydrocortisone and Calamile lotion over the counter with no improvement. I went to the doctors, he prescribed a steroid, the rash lightened however back pain started, accompanied with stomach aches. Now I have a heaviness on my chest that hurts. SHE STATED THAT SHE FELT FINE AND DID HAVE A BIT OF A SORE ARM AROUND THE ADMINISTRATION SITE. SHE STATED THAT IT FELT LIKE HALF OF THE PAIN OF A TETANUS SHOT. SHE DID TAKE A GLASS OF WATER AND STATED THAT SHE HAD RECENTLY EATEN. 911 WAS CALLED AND THE PATIENT EVALUATED AND WALKED OUT OF STORE. NO MEDICATIONS ADMINISTERED A TIME OF EVENT.

Fever, chills, vomiting, fatigue for 2 days following the 2nd dose. On Sunday Feb 28th she had cellulitis to the injection area that was red, swollen and getting larger. . She went to UTC and was given antibiotics for cellulitis. She then had fatigue for the week of March 1. On March 20th she went to UTC for sore throat and enlarged tonsils. She was told that she did not have strep. No other diagnosis at that time. She continued to have fatigue and sore throat for the week March 8th. On March 14th, she went to UTC with sore throat, tonsils were visibly enlarged and blotchy. She was diagnosed with Mononucleosis.

rash, papular on the entire body sparing the palms and soles. started 2 days after vaccine. no other symptoms

Localized swelling since injection first administered. About 7-8 days later redness, itching, and fever began at sight of injection swelling.

1 week after recieving the first shot of the Moderna vaccine, I bumped my right arm about 2 inches below the injection site. Throughout the course of the day, my arm progressively became swollen and sore, to the point where I couldn't move it much or sleep on it. The next day it got progressively better on its own, and then I began taking an antibiotic prescribed by my doctor that helped it heal the rest of the way. I'm not positive if the rest of the healing was on its own, or directly because of the antibiotic.

A smaller rash started the day after I received the vaccine. about twelve hours later, the rash grew another 3 inches outward, I had been monitoring it with marker. it might continue to grow, or not.

Pt has chronic pain on her shoulder from her car accident in September 2020; however, after the vaccination, the same where she was feeling pain was also the site on injection. On 3/12/21, she started feeling jaw pain on same side of injection and shoulder pain intensified plus numbness. Patient says the pain and numbness comes and goes. She is due for her next dose on Saturday, March 28 but decided to wait a little bit longer before she gets her second dose.

About 5 to 7 minutes after receiving shot (while waiting her 15 minutes post vaccination) she came back up to pharmacy window and said she was getting hot and red all over and felt she was having allergic reaction. I sat with her for about 30 to 40 minutes to make sure she was maintaining normal breathing. She did take some oral Diphenhydramine as was very itchy and red. She did have friend with her and we instructed her to go to ER if got worse. Next day 3/24/21 patient called and said she had a productive cough and was advised by staff pharmacist to go to Dr. She called back and said did go to Dr. and was prescribed steroids and antihistamine (was on steroid at time of vaccination but we were unaware) and Dr told patient not to get 2nd vaccination. Patient was doing better but not completely recovered

Left arm swollen, red, hot, itchy, fever of 100.4, 1 day, generally felt bad. The redness, swelling, itchy, and feeling bad lasted 2 weeks. Treated with benadryl and ibuprohin.

Hives that itch and burn all over my body that disappear and come back within hours. This is first time I have broken out in hives. Diet and environment are unchanged. I?m on immunosuppressants I don?t know if this would effect or be related to my hives. I?m on day 3 of having them and I?m not sure if it?s related to the vaccine. My doctor prescribed a weeks worth of steroids to see if it would have any affect of the hives.

Lingering pain in arm that hurt to move, red area with the width of a dollar bill. Redness decreased by Thursday, but switched to a persistent itch. Redness was even toned and warm, skin felt slightly rough.

Slight headache and fatigue by 7:30 that got increasingly worse. Muscle aches throughout whole body, especially around injection site. Extreme fatigue, muscle soreness and headache through the night and into next day. Woke up feeling like a terrible hangover. Took ibuprofen at 8am. Slept all day. I received a rash at the injection site within a couple hours after receiving the first dose of the Moderna COVID-19 vaccine. The rash worsened after my second dose 4 weeks later. It has now been almost 8 weeks since the rash appeared, and it still has not gone away. It looks like small red and yellow bumps covering my upper arm. There is no soreness or itchiness associated with the bumps, but they do seem to improve but then return.

Pt c/o fatigue, malaise, left arm/injection site soreness, sweats, and dizziness. Decadron 8 mg IM administered. Rx given for Medrol pack. Rests. Ice and gentle massage to left deltoid recommended. OTC Benadryl and Ibuprofen prn symptoms recommended.

Patient was very tired on Monday, 3/30/2021 and took a nap. When she woke up, she was covered in hives on her face, down her neck, all over her back and down her arms. The hives up her spine were the size of quarters. Immediately gave her 2 mg tabs of Benadryl and used Caladryl lotion to control the itching. After 1 hour, Patient took a cold shower to stop the spread of the hives and re-applied additional Caladryl. Maintained the Benadryl dosing throughout the rest of the day into the next day continuously. As of this writing on 3/31/2021, Patient still has hives on her arms.

Moderna COVID-19 Vaccine EUA bad rash all over body (hives) sore throat fatigue swollen fingers, wrists, eyes, ankles

Lupus; Obstruction breathing; Lips were swollen; Hives on face/ huge welts all over body; Fever; A spontaneous report was received from a consumer concerning a 20-year-old, adult female patient, who received Moderna's COVID-19 vaccine (mRNA-1273) and experienced lupus/systemic lupus erythematosus, fever/pyrexia, hives on face/huge welts all over body/urticaria, lips were swollen/lip swelling and obstruction breathing/obstructive airways disorder. The patient's medical history was not provided. Concomitant product use was not provided by the reporter. On an unspecified date in Jan 2021, prior to the onset of the events, the patient received the first of two planned doses of mRNA-1273 (Batch number: Unknown) for prophylaxis of COVID-19 infection. On an unspecified date in Feb 2021, Patient received the second of two planned doses of mRNA-1273 (Batch number: Unknown) for prophylaxis of COVID-19 infection. On an unspecified date in Feb 2021, within 24 hours after receiving the second dose, patient experienced fever, hives on face/huge welts all over body, lips were swollen and obstruction breathing. The patient went to the ER and was given unspecified steriods and Diphenhydramine and the patient went to the ER (emergency room) on 19 Mar 2021 because she had another reaction and the doctor confirmed her that the vaccine triggered lupus. The event lupus was considered as medically significant. The patient received both scheduled doses of mRNA-1273 prior to the events; therefore, action taken with the drug in response to the events was not applicable. The outcome of the events, lupus, fever, hives on face/huge welts all over body, lips were swollen and obstruction breathing was unknown at the time of this report.; Reporter's Comments: Based on the current available information and temporal association between the use of the product and the start date of the event, a causal relationship cannot be excluded.

MODERNA EUA--The patient complained of having a rash on her face the day after receiving her first Moderna dose on 031621. She denied any fever, itching, shortness of breath or wheezing. She was also noted to have some bruising under her left eye. The initial encounter with the healtcare provider occurred two days post-vaccination via virtual visit on 031321. She was followed up in person on 031921 at which time her rash was improved after taking Benadryl. She reported some pain of her lower left eyelid that continued but no vision changes. Another virtual visit was conducted on 032621 at which time all symptoms had resolved without further sequelae.

Patient presented at 14:17 with urticaria on right forearm, pruritis on back, trunk, bilateral arms, scalp and facial flushing. Denies shortness of breath, chest pain, headache, difficulty swallowing and difficulty breathing. 14:22 BP 122/78, P 86, O2 99% RA, R 18. 14:24 administered 25 Mg Benadryl PO. 14:30 BP 116/76, P 81, O2 100% RA, R 18, patient reports improvement of symptoms. Advised patient to seek medical attention if symptoms worsen, additionally advised patient to consult with allergist prior to 2nd dose administration, patient verbalized understanding. Patient left at 14:45 with boyfriend in stable condition, no respiratory distress, patient refused medical transport to hospital.

On the second day in the afternoon I noticed a rash below my neck and above my waist. The only other symptom I had was a headache.

After receiving my shot on the 26th, my arm was sore as is standard for vaccinations. I was fine the next day. When I woke up on the morning of April 3rd, my arm was swollen, sore, and itchy around the injection site. I tried taking some acetaminophen to help with the swelling and soreness, but it didn?t change. Icing it helped with the swelling.

Exactly eight days after taking the vaccine my arm became really sore again and the lymph nodes under my arm swelled up and are painful to the touch. I have a rash on my arm like a red welt, but it does not itch, it is just sore. On the affected arm my hand shakes and it feels weaker than my other arm.

I first began to form a rash where I received my vaccine. Over the course of an hour, it quickly began to swell and turn red. It was very sore, sensitive, and hot to the touch. By the end of the day, a raised lump formed and was about 4 inches in diameter. The swelling lasted for about 3 days. I then decided to take Benadryl to help with the itchiness and swelling. Since taking that medication, the swelling has significantly gone down but is still present.

Moderna COVID-19 Vaccine EUA A week and a day after my first dose, a rash at the vaccination site (upper left arm) formed and is the size of about two quarters and a penny. The rash is raised and extremely itchy. I have not used any new products on my skin lately and this is the only area of my body where a rash has appeared.

Patient developed red/pruritic rash and induration to the right deltoid region. Induration is improving but rash persists.

Pt received her 2nd dose of the Moderna COVID19 vaccine and after sitting in observation for 20 minutes reported feeling itching around the injection site. Pt has a red rash above the injection site about the size of a half dollar coin. EMS was called and came to evaluate the pt. Pt's symptoms did not progress, she felt fine otherwise. BP 126/98 repeat 130/84, HR 130 RR 14 O2Sat 99% By the time EMS arrived the pt's rash started to subside. Pt signed refusal for transport to the ED with EMS. Pt was given ED precautions.

Pt has diffuse rash to the bilateral upper and lower extremities in addition to the face. Sparing the trunk. Pruritic, painful in nature, many had a vesicle in the center.

Patient received the vaccine @ 1510. At 1520, she developed a huge rash and hives on her right arm. She reported "not feeling good". Placed patient on a cot with head raised at 45 degrees. She was massaging her neck, saying there was a "knot" in her throat. Shallow, rapid breathing @ 25-30 per minute, and swelling of the lips noted. BP=130/80, HR @ 95-101 per minute. @ 1522, Epinephrine 0.5 mg. administered at L vastus lateralis IM. 911 was called per protocol. At 1533, patient reported no relief and swelling of the throat noted. Placed in the left lateral position. At 1539, EMS arrived. She was wheeled out via stretcher to be taken for further evaluation and treatment. Patient was conscious and coherent when she left the building with the EMTs in an ambulance.

My whole body started feeling sunburned about 8 hours after getting the vaccine. My skin was very tender all over. There was no rash.

Patient experienced itching following administration of vaccine. 50mg Capsules of Benadryl was administered & we monitored her for 45 minutes. Itching was only slightly relieved so employee, had a contact at Hospital & called Dr. Dr said to bring patient over to office & he would examine her. She was actually a patient at office currently. Dr said she was having an allergic reaction & conitnue to take Benadryl around the clock.

Fever started at 11:30pm 4/6/2021. Fever ~100 degrees F. Had chills and couldn't get warm, then was hot and drenched in sweat. Repeated all throughout the night (Hot/cold). Took 500mg Tylenol at 12am that night. Woke up at 2am with a ~102.4 F fever. Took another Tylenol. Got out of bed around 8am 4/7 drenched in sweat. Felt nauseous and vomited almost right away and couldn't stop for a few minutes. Still had around 102 F so took 2 more Tylenols. Throughout that night and until Friday, I was achy all over my body. Also, lymph node under left arm pit was swollen and was sore along with my left arm. Throughout the day on 4/7, laid in bed trying to get rest and too nauseous to eat anything other than some cereal in the morning and plain noodles later in the day. Took another Tylenol in the afternoon because I was still staying at 102 F fever. Was slightly dizzy when standing up or moving too fast. I slept the next night and felt fine 4/8 morning (no fever), but felt worse as the day went on, aching everywhere and feeling more and more dizzy. Friday 4/9, I woke up in the morning and when I first started walking, I fell on the floor because of extreme dizziness. The whole day I was extremely dizzy and I've never had vertigo before, but it seemed like that based on explanations of it. Also had painful headache every time I stood up, but was fine when I wasn't moving. The next day it was mild dizziness and completely gone the day after that. Also, Thursday 4/8 around 7:30pm, I noticed I had hives on the back of my right hand. Not 100% sure if it's due to the vaccine, but I'm not sure what else would have caused it and it's worth noting. It was gone the next day.

Patient was sitting in post-vac when her color began to change to a reddish tone. Upon RN's arrival, patient stated she was starting to experience symptoms of nausea and dizziness. Upon further assessment, the patient stated she did not eat or drink anything. She also stated her last meal was last night (4/12/21) at approximately 10pm. Patient was given water and advised to drink slowly. Educated patient on signs and symptoms of hypoglycemia. Patient stable and departed at 1053a. Patient was vaccinated at 10:25am, began experiencing the symptoms at 10:35am. This was the patient's first dose of Moderna 0.5ml. 10:30am B/P 112/70 Pox 99% RA HR 59 10:49am B/P 116/65 Pox 98% RA HR 63

On the day of the vaccine, I had pain in my arm where I got the shot hours later. I had a bit of a fever and chills that night. The next day I began to get bumps on the arm that I got the shot at around 2:00 pm. I took an allergy antihistamine pill and it went away for a couple of hours. Later at night, I noticed small bumps and went to sleep. I woke up at around 6:00 am (2 days after getting the vaccine) and noticed I got hives all over my arms and legs. I went to urgent care and they said I should just continue using the antihistamines. I did and it has been two weeks since the vaccine and every day I still get hives. Not one day has passed where I have not gotten an allergic reaction with hives.

2 days after second dose of Moderna I got random bruises and hives all over my body and a week later it?s still here

My arm was extremely sore and I had a fever for the following two days after the vaccine but then it went away. I was feeling back to normal until yesterday, my arm got very red and swollen and today it?s hard, hot, and hurts to the touch. I?ve also been extremely tired the last two days

Received the vaccine at 1230 and felt fine. Got home and felt tired, napped for two hours and got up for 30-45 minutes, went back to bed. Started feeling like she was spinning in bed and felt light-headed. Arm felt sore when she went back to bed. Today, April 14, 2021, woke up and looked in the mirror and was surprised to see a red raised area, sense to touch, painful. Noticed heart was beating quickly, and "feels off." Swelling is noted on the left deltoid measuring 60mmx36mmx2mm elevated and red, tender to touch.

Severe skin rash covering the entire torso (stomach, chest, neck, back, shoulders), and both forearms that started 1 week after the 2nd dose. Two different doctors diagnosed it as Pityriasis rosea, and prescribed a 12-day regimen of Prednisone. Rash looks like red, raised bumps all over the skin.

Pinpoint red round spots on insides of both arms below elbows appeared with no itching appearing on 4/12

Swelling occurred at the injection site. It then welted up and got larger the next few days. It was red and itches but also hurt. Couldn?t lift my arm past my shoulder. Not other symptoms occurred.

Began having chills and fever that night. Throughout the night, I could not sleep and was in an out of body experience. My skin was on fire all over my body and I couldn't even stand to have clothes on me. Had the worst headache for the day after.

Around 11:00 in the morning I was starting to feel weak and hard of breathing with a sore arm where the injection site was. As the day went on it progressed to muscle aches and a low fever, I went to bed with chills, muscle pain, heavy breathing, and a 100.8 fever. I work up in the middle of the night to go to the bathroom and felt nauseous, I don?t remember much but I remember waking up on the floor in a cold sweat, so I assume I passed out from the nausea. I went back to bed and I woke up with a 101.8 fever and weakness and muscle pains. My mother told me to take Tylenol every 4 hours and be on top of things, my symptoms have been decreasing since this morning but as I am filling this out at 11:00 at night I?m slowly getting my fever back. I will add this is exactly how I felt when I got covid back in November of 2020.

3/24/21 Incoming Triage Nurse call: Hospital called and said patient is there now and had got her 1st Covid Vaccine- Moderna and she did get a small rash on her left forearm that does itch and is red. Nurse said patient got the shot in her left arm and got the rash on her forearm a few minutes later. 3/26/21 ER HPI: dx: urticaria 20 year old female who presents with ITCHING IN LEFT FOREARM. GOT COVID SHOT ON 3/24/21 AND HAS HAD INTERMITTENT ITCHING ON LEFT FOREARM SINCE. PT DENIES SHORTNESS OF BREATH AND CHEST PAIN. PT STATES WHEN TAKES BENADRYL GETS BETTER AND THEN RETURNS. PT STATES SAW Doctor AND WAS TOLD TO GO TO ER IF IT DID NOT IMPROVE IN 24 HOURS. Triage nurse: Presents to the ER from home with complaints of itching and redness to left lower forearm. States she had the Covid injection (first one) on 03-24. States had taken Benadryl after the injection to help with itching. States "redness comes and goes." Complaining of headache, 8/10 pain, constant throbbing with onset yesterday. States Tylenol did help alleviate pain some, not completely. States she was instructed per PCP to come to ER if redness, itching continues

Small bumps/hives initially appearing only on the opposite arm of vaccine site where the arm bends. The bumps later appeared on the arm which received the vaccine and in small numbers on the chest. The raised bumps are red and itchy.

48 hours after Moderna vaccine [ given at outside facility] patient started with puffy eyes, rash around the eyes with redness of both eyes. Slowly the rash spread to entire face including the neck region. At 72 hours post vaccination she noted swelling and redness around the lips with tightness in chest prompting here to seek help. Noted to have generalized swelling and erythema to face along with the periorbital area and eye lids on exam [ after Benadryl]- no respiratory distress. Reaction thought to possibly be an acute worsening of her underlying eczema but could not rule out vaccine reaction. Treated with Benadryl, Zyrtec and Prednisone

started off with itching- then redness and swelling at injection site. NOw more intense itching- will go and get benedryl OTC and cream and reck in 2 wks

after my initial injection swelling and rash went away but about three days after no side effects i had a rash that was about three inches long and one inch wide with a huge mass underneath. four or five hours after the new rash appeared, bruising and a burning sensation started to occur but the rash significantly reduced.

Moderna COVID-19 Vaccine EUA At 12 pm I started to feel tired. As time went by I got more tired as I was cleaning things. I could not do anything for a long time. I laid on the floor for a while and watched videos. I felt better once I did that. I got tired from sitting and laid on the floor. It went until 4 pm April 24 2021. I got up and was fine after taking a nap on the floor. April 24, 2021 I got a small headache that was there and now comes and goes with longer time periods. From the time I got the shot 10:43am, I felt a headache after it was done to 5pm. It is now going away at 6pm. I feel pain at the place of shot even more at 3pm it started now at 6pm it has increased. It is bearable.

Bruising and swelling was gone after three days post-shot. I woke up eight days post-shot and the injection site was red, itchy, and there was a lump the size of a small orange.

Around 9:45p my face and neck broke out in hives, swelled up, was extremely itchy, and hot. Shortly after it became hard to swallow as my throat was closing. I went to Emergency Department around 10p. The doctors administered Epinephrine (via Epi Pen) and Pepcid through an IV. I was prescribed an Epi Pen as well as Claritin and Omeprazole for 14 days.

Facial rash that spread to upper extremities bi lateral, along with nausea. Client transfer to ER via EMS for further evaluation

Beginning approximately 12 hours after the shot the following symptoms occurred and became progressively extreme throughout the night, causing an inability to sleep until the next day; Sweating; Fever; Headache; Chills; Nausea; Fatigue; Inability to sleep due to extreme side effects; Pain at injection site; Pain in arm muscles; Decreased arm mobility; The day following the shot the symptoms continued at an extreme intensity, until becoming less extreme but still disruptive in the evening. That night I was able to sleep and the next day the fever, sweat, and chills were gone. That day the rest of the symptoms continued in a milder way and by the day after I was feeling nearly normal again.

Patient received the vaccine on Thursday, 4/22/21. Patient started having warmth and redness at injection site on Tuesday, 4/27/21. Patient hasn't take any over-the-counter medications. Patient stated the the swelling has increased. Patient stated the pain has stayed the same. Patient reported side effect to pharmacy on Friday, 4/30/21

Dyshidrotic Eczema 8 days after vaccination. I have never had a previous eczema reaction in my life before. It began with small red bumps on the palms, then on elbows, then on feet. It would progress slowly day by day. The small red bumps started to look like blisters.

I started to have blisters on the palm of my hand. Most blisters are on my left hand. Few on my right hand and one on my right feet. I also have rashes on the back of my lower thighs.

Rash underneath injection site - red, raised, itchy, 2.5 inches in diameter - appeared the day after the 2nd dose, assuming it will go away in a few days, but thought to report it just for records.

The day of getting my vaccine, I had received the shot at 1:30pm on 4/30. I had no symptoms besides a sore arm at the injection site. I woke up at 4:00am on 5/1 with a fever of 99.9F and chills. I decided to take a hot shower because I couldn?t get warm. At the end of my shower I suddenly got a ?sickly? feeling in my chest, similar to symptoms of a chest cold. My vision started going black, almost like when you stand up too quickly. I experienced a sudden throbbing headache out of nowhere. My ears were ringing with partial hearing loss, and I started getting very nauseous. I stumbled out of the shower and immediately vomited.

Menstruation started two weeks early (approximately one week after vaccine) and lasted for 14 days, varying between light and heavy throughout the two weeks. Also had high fever (reaching 103.8) for four days (starting two days after vaccination) which was reported, and a rash of purple spots across my chest, arms, and back started after fever stopped. Rash was not very bothersome, but alarming. Experienced frequent nausea and lightheadedness, but I also was experiencing those symptoms at the time due to my concussion. One week exactly after vaccination all symptoms were gone and no treatment was needed.

Patient received vaccine 5/2/21. Patient went home and noticed swelling, redness, bruising, burning, and numbness (to end of fingertips) in the injected arm. Reported severe dizziness and passed out. She also experienced increased heart rate/blood pressure. On 5/3/21, while experiencing the same symptoms, went to nearby medical facility (urgent care) where she was told that a nerve had been hit or bone had been hit. She was sent home with a heart monitor, in a sling, and was told to stay home from work for 2 days. Her symptoms continued through the day, continuing into 5/4/21. As of 6pm 5/4/21 she is going to go to an emergency room.

Pt experienced a localized injection size reaction, started with a quarter size red bump and progressed to a welt as large as a fist. The patch was red and hot to the touch and itchy. The pt was referred to primary care for further evaluation, and recommendation as to whether pt should receive the second dose in the series.

After the first shot, April 8th, about two weeks of receiving the first vaccine, I had noticed a bump of my left side collar bone and I got an ultra sound for and it was just an enlarged lymph node. A day after the second vaccine, May 7th, I noticed that the shot area was red, warm and enlarged, with itchiness. Also the left side of my neck where the nodes are, is swelled and tender to the touch.

A week after receiving the vaccine I noticed a large oval shaped redness around the vaccination site. It is extremely itchy. The swelling and soreness I initially experienced had mostly gone away by this time. I have not applied any medications to the site due to not knowing if I should. I have not visited a doctors office, clinic or emergency room due to not having health insurance. The doctor I listed as the one to contact is who gave me the vaccine. The red ring as remained all day and has not been getting better so far.

Pain at injection site spread through entire arm and through upper shoulder into neck. Pain was so extreme that the arm was immobilized- any movement no matter how small caused pain to flare. It felt like a broken bone. Injection at 2:30pm, pain peaked around 10pm. Next morning awoke with same extreme pain and added severe body aches in lower back and legs along with a headache. Pain persists as of report

My arm got red and swollen about 30 mins after. It's been like that for like a week and a half now. It's less red some days and more red others. It has not spread

Patient presented to pharmacy on 05/12/21 for second Moderna dose. While filling out VAR, she noted that she did have an adverse reaction to the last dose. When asked, she stated she developed a red, bumpy rash all over the side of her body about 4-5 hours after receiving the vaccine. She declined itching, noting that it was just red and swollen, and took ~5 days to go away. She declined shortness of breath or difficulties breathing, but did note her throat felt tight. She did not take any medications and had not mentioned it to anyone until now. Due to the non-severe immediate allergic reaction, the CDC recommends not giving the second dose. Recommended the patient does not receive another dose.

Intermittent lymphadenopathy of the upper body (pre- and post auricular, submental, cervical, axillary, occipital). Fatigue, ear pain, injection site pain still present at day of report. Treatment for intermittent pain has been NSAIDs PRN and Wellbutrin for the fatigue.

12-13 Petechiae on left forearm onset 12 days after second Moderna dose. 1 on right eyelid onset day 13. Blood work showed normal platelets. No other symptoms. Petechiae began fading 1 day after onset. Eyelid petechiae still present.

Chills, then fever (101-103F) from approximately 1:30 am to 6:00 am. Awakened around 1:30 am nauseous and hot to touch.

Large red painful welt at site of injection Extreme cold chills, to the point of feeling like needles were in my skin Body aches that confined me to my bed and did not allow me to move

A rash occurred about 6 weeks after the second dose. It may not be from moderna vaccine but I was advised to report it. The rash began on the chest with 5 dots in a circle pattern, then spread up to the neck and sparsely down the body. Did not itch for the first few days but began itching on the 3rd day.

On 5/17/21 the Patient received the Moderna Covid Vaccine. She presented with a known allergy to PCN and Augmentin. This was not a reason for the patient not to get her vaccine today so Pharmacist proceeded to give her the shot. We asked for the patient to sit down and since she had some allergies we asked for her to wait for 30 min. Roughly ten minutes into her wait the patient started to complain of numbness of the face, so we went ahead and gave her a cold bottle of water. About two minutes later the patient began to complain of numbness in the arm along with hives on both arms and neck. At this point the patient looked very flush, disorientated, and acted as if she was going to pass out. I went and sat down with her and I could tell she was uncomfortable for which I got her some ice packs and started asking more questions about her reaction. I could tell the patient was very disorientated and she began to complain of swelling and tightness of the throat. Once the patient began to complain of throat swelling/tightness. I had call code white and 911 due to the concern of anaphylaxis and pharmacy protocol.

Experienced ?COVID arm? with hard, red splotchy spot on the injection site over a week after first dose

Patient reports received 2nd dose COVID Moderna vaccine around 11am 5/14/21. A few hours later, noticed red bumps (not itchy) on abdomen. Rash on abdomen, buttocks, and back mainly. After 1st dose, only adverse effect was sore arm. No rash.

HORRIBLE unbearable arm pain and chills throughout my body i would not be reporting this if the pain wasn?t this bad i feel scared and someone nervous because i can barely use my left arm where the vaccine has went in. I got the vaccine on Monday 11:20 am and then after was fine and normal then i went home and worked out at around 4:30 and after that the pain increase but it wasn?t until the Tuesday which is the day after i got my vaccine that i could not function with out ibuprophen and the second that it wore off the vaccine site was hot and the pain was radiating throughout my arm making it near impossible to do anything it is now Wednesday and the pain has gotten way worse and i am unsure of what to do other than to force myself to take Advil so i can function

I had no issues with the 1st dose, except for a sore arm. The person who administered the 2nd shot seemed to have injected it way too fast, I noticed a burning sensation as it was injected, and I also noticed that when the needle was removed, the medicine was dripping down arm. A few hours after receiving my shot (5:30 pm), I started to notice a bad headache, feelings of exhaustion, nausea, and hot flashes. I checked my temperature and it was 99.6 degrees, which is higher than I normally run so my body was doing something. I noticed a hard, swollen, red bump at the injection sight on my right arm - which has progressively gotten worse as the days have gone by. I still have the low grade fever, exhaustion, headache, nausea, and swollen/sore arm.

Two days after the vaccine, I developed a rash on my arm and severe pain. Then the day after I was having horrible chest pain and heart palpitations. I was given heparin directly to my heart to treat a possible blood clot.

The following wednesday after receiving my covid vaccination I was getting in the shower when i noticed small red raised bumps all over my body. They were most prominent on my midsection, arms and upper thighs. Though the bumps were alarming most of y itching is in paces I don't see the rash such as my hands and feet. However, some of the bumps do itch as well. I've been treating at home with 24 hour claritin, and anit-itch creams. These remedies supress the intensity of the itch, however I am always somewhat itchy. Having had these symptoms for almost a week now the itch is beginning to lift somewhat, but not completely.

I have a red and itchy blotchy rash all over my torso that started from my vaccination site and spread. It started spreading slowly but it is spreading more rapidly now and is starting to spread lower onto my legs and bikini area as well. It is super itchy and painful, and more and more just keep popping up on my body at alarmingly increasing rates.

In the beginning, I had my arm ache, and in the afternoon I had a headache. The next day my arm was sore and hurt. By the 23rd of May, the place where the injection took place was red and swollen. I still had small pain. On the 24th, it was still red, a bit swollen, and felt a bit itchy, I put on some Vaporub. Yesterday, the 25th, I had a mild headache and the injection site looked like a small bruise but with a green color. Today, 26th of May, the bruise is yellow-ish and still itches.

one day 5 feet swelled with a red, itchy rash and were painful to stand on. This lasted 5 days. Currently, lymph nodes in neck are swollen and painful.

A few minutes after receiving the vaccine the patient experienced color change/purple hives on from her neck up through the face and ears. Patient become short of breath and weak and was laid to the floor. Pharmacist was immediately notified and he assisted in positioning her and immediately assessed the situation. He determined that an epi-pen was appropriate and he retrieved and immediately administered one dose. Very quickly the reaction was reversed and patient color was restored and situation stabilized. No second dose administered. EMT arrived and did full vitals workup. She left on her own power per EMT group and went to the local hospital for observation. Pharmacist followed up after 2 hours and patient is doing well.

Patient presented to the ER after developing pain and itching at injection site after COVID-19 vaccine. Pain was worsening and redness was expanding. Patient was diagnosed with early mild cellulitis and treated with Keflex 500mg QID for 10 days.

Hardness at injection site, tenderness around entire shoulder, red splotchy rash, heat in rash, swelling. Started a couple of hours after vaccination

My arm around the vaccination site is very red and swollen and sore. And where I got my shot it is badly bruised.

Pain and tenderness in the surrounding area of the injection site. The muscle soreness in the arm started about an hour after receiving the vaccine and worsened into the evening. The tenderness mostly went away by the next morning, the worst of it being the evening of the dosage. Tenderness and slight swelling to the lymph nodes in the same arm as the injection site started in the early afternoon the same day as the dosage. This tenderness lasted for about 5 days. Experienced fatigue and a mild headache the evening of the dosage.

Patient reports significant swelling at injection site with some pain/sensitivity which began approximatley 1 - 2 days after vaccination

PATIENT REPORTED PAIN AND INFLAMMATION IN HER ARM 10 DAYS AFTER 1ST SHOT OF MODERNA. SITE ON LEFT ARM WAS RED AND INFLAMMED. PATIENT REPORTED HER ARM WAS STILL VERY SORE (SCORE 4 ON 1-10 SCALE) BUT REPORTED SHE FELT HIGHER PAIN 2-3 DAYS AFTER INJECTION WAS GIVEN. PATIENT ALSO REPORTED THAT SHE HAS BEEN TAKING TYLENOL FOR THE PAIN. SHE HAS ALSO BEEN HAVING HEADACHES THAT STARTED AFTER RECEIVING THE VACCINE. SHE REPORTED THIS IS ABNORMAL FOR HER AND USUALLY DOES NOT HAVE HEADACHES. SHE HAS BEEN DRINKING LOTS OF WATER SO THE HEADACHES ARE UNRELATED TO DEHYDRATION.

Patient received 2nd dose of Moderna vaccine today (6/12/21) and experienced shortness of breath, raspy voice, visible redness/swelling to face, difficulty speaking approximately 1-1.5 hours post vaccination. She received the 1st Moderna dose on 5/14/21 and did not experience any significant adverse events (only experienced arm pain, redness/swelling at site of injection). Administered epi 0.3mg IM x1, diphenhydramine 50mg IV x1, Solu-Medrol 125mg IV x1, famotidine 20mg IV x1, albuterol 5mg neb x1, and NS 1L bolus for anaphylaxis treatment in the ED. Also O2 4L/min per oxymask.

Patient presented to the ED, per provider notes: Patient presented to the emergency room with hives to bilateral hands and feet one on her right upper thigh and some on her right arm. She reports on 7 April she received her initial covid vaccine. She notes on April 15 she started to develop hives around the injection site and down her right arm.

LT arm got swollen Friday morning and throughout the day it started to increase. It felt hot with a sting to it. Saturday AM It was hard to breath and I was unable to take deep breaths since I felt like my inside were really swollen/Inflamed. Sunday AM the swollen had got down leaving a bruise as big as the inflammation had been.

Woke up on March 12 with itchy bumps all over both hands and feet. They then bloomed into a rash known as Erythema Multiforme. It is now July and I still get small bumps that appear on my fingers.

Vaccinated Tuesday 07/06/2021 at 12:00pm. At 12:15 I was having palpitations, and my throat felt tight, I attribute this to anxiety, I was nervous to get the shot but think it should still be noted. At 1:00 or 2:00 that afternoon, I noticed about an inch around where the shot was injected I had small white head like bumps, that if scratched or pressured drained white puss. At 3:00pm that afternoon, I stood up from my bed and felt pressure in my head similar to sinus pressure you can get from a cold. I had no congestion. On Wednesday 07/07/2021 I woke up and my upper L arm was in significant pain. The area that was vaccinated had a 2-3 inch radius that was bright red in color. The area where I was injected had a 0.5-1 inch radius that was dark purple. This area was hot to the touch, firm, and swollen. The pain was constant but worse when touched. On Thursday 07/08/2021 there was no new symptoms and the skin reaction started to go away but was still present. Today Friday 07/09/2021 the skin reaction is still present but improved. It is still painful to the touch. The redness is gone but the dark purple circle is still pressent. It is still slightly firm, and is no longer hot to touch. It is still slightly painful to touch but not bothersome when left alone. Today I had to leave work early due to a drop in blood pressure at 12:00pm. Around 10-11am I was standing and typing when I felt like I had come down with the flu. This happened rapidly within 2 minutes. I felt feverish, I was sweating, I felt nauseous, but I did not vommit, I felt as though I was going to faint. I had one of the nurses at my workplace check my blood pressure. It was 107/72. Within an hour it rose to 108/72 so I decided to go home. It should be noted I do not normally eat breakfast. I stay well hydrated. I had only had my medication, and water at the time of this event which is normal for me. I was given crackers, and juice that did not help. I am unsure if this is related to the vaccination but think it should be noted.

One week after the first dose of the vaccine I started to get hives that spread all over my legs. The next day the rash was on my stomach. Every day they went from the soles of my feet and worked their way up my body until it reaches the palms of my hands. After this point I have been experiencing facial swelling. After the second dose the hives started again but it was much worse. It started the day after and I also had swelling in my face and lips. I have been referred to an immunologist. I still have hives every day after five months. They are mostly on my legs and thighs. They are big raised splotches which cover large areas. I have never had them on my face since the day after the second dose. I have taken Steroids and I still take Zyrtec every day. It is extremely itchy which was not helped by a topical cream. I also take anxiety medication so that I can sleep. Dose 2: 02/05/2021 LOT#012M20A

My symptoms started one minute after receiving the vaccine. It started out I was itchy where the shot was. Then my whole body became itchy. The pharmacist gave me 200mg of Benadryl. It helped but not completely. I went to the doctor within a couple of days to try and get treatment for the itchiness. They prescribed some kind of steroid but I did not take it because I did not want it to prevent the vaccine from working. The itchiness lasted about one month. It was very intense and I had to take Benadryl the whole month. I did see an allergist that told me it was a case of hives, but the hives do not show.

While receiving my second shot, a drop of the vaccine liquid came into contact with my arm, dripping down. I brushed it off, as well as the pharmacist. A day later the vaccination site started to hurt, which was expected but also itch. On Thursday, two days after I received my shot, my friends pointed out that my arm is in hives, red, and swollen. My arm is still in hives, and itches like crazy. It looks like an allergic reaction. I have pictures

Pain and swelling at site of injection, fatigue, chills, severe body aches, headache, and a low-grade fever.

1 week after receiving the vaccination, a red welt appeared on my arm where the injection was given. It was hot, raised and itchy and about an inch and a half wide and long. I drew a line around it. The next day it was less itchy and hot but had gone about half an inch outside the initial lines. The third/ fourth day it was barely raised at all but had gotten even larger, now about 1.5-2inches outside the initial lines in some areas. It is now barely raised, barely red and has not gotten any larger since yesterday.

Started with arm pain, then lymph nodes in armpit were swollen and arm pain increased, could barely move arm because of pain, nausea and vomiting, chills and fever of 100.9, injection site the size of a baseball almost 48 hours after injection, super fatigued

At 11:30 pm my left arm was very sore, overnight i couldn?t sleep because of an achy pain and swelling in left shoulder, morning came end i tried to move it with terrible pain, redness is now visible (11:30 am 01/05/2021), pain and swelling are still persistent.

sudden itchiness on area surrounding injection site at night after 1.5 week of injection and continue to persist for the following days, a small red bump, the area around the injection site the size of a small child?s palm is pink/light red in color

Night of vaccine, red raised itchy rash around injection site, foggy head feeling, left arm pain. All of these effects went away in 2 days. 8 days following administration of first Moderna dose (1/5/2020), the raised, red rash around injection site had returned (same size as before), with nausea and vomiting. Slight headache.

Patient was awoken about 6:25am with severe pain at injection site, sweating, shaking all over and nausea. Patient took 4- 200mg ibuprofen at 6:30am. Patient fell back asleep and awoke at 8:40am. Patient stated that arm is still in pain, still sweating. Nausea and shaking has gone.

1/5/21-About an hour after receiving vaccine, I had mild nausea, my left arm kind of hurt after receiving the vaccine.. which is weird because I got it in my right arm. 1/5/21- Around 8:00 PM I noticed raised red bumps on both of my forearms.. it?s worse on the right arm. It looked like a mild rash. Around 10:00 PM I developed chills, muscle aches, my right arm was in a lot of pain, I had a slight headache, nausea, and my temperature kept going up. I?ve been taking Advil/ Tylenol to try and get my temperature down.. but after every dose wears off my temp goes back up. I kept waking up multiple times in the middle of the night. In the middle of the night around 2AM, my temperature was as high as 102.9. Later on in the night around 4AM, my chills had stopped and I actually felt really warm. 1/6/21- I woke up about 6:30 AM and had to call out of work. My temp was about 98 something at this point and I still had muscle aches. I slept most of the day and my temp continues to go over 100 now every time a dose of Advil wears off. I still have muscle aches and there is still a lot of pain at the injection site. My arm doesn?t hurt as bed as it did though. However, the rash on my forearms is still there. On my right forearm it looks worse than it did yesterday.

patient experienced rash post vaccination. patient also has raised red welt on arm that is still present 1 week post vaccination and has not improved. patient does report pain in the area but states that it is warm to the touch. patient continue to monitor for resolution and will update staff. patient stated she took advil - for headache but did not see improvement in arm redness. patient declined medical attention from the emergency room today.

Fatigue since an hour after injection, muscle pain since an hour after injection, site soreness sense 30 min after injection, chills for 45 minutes starting an hour after injection, and nausea for 45 minutes starting an hour after injection

Localized redness in a circular shape at and surrounding injection site. Along with redness, the area of skin is itchy, raised, and hot to the touch. Symptom developed 7 days after receiving dose 1 of the COVID 19 Moderna vaccine.

Lightheaded, dizziness, body aches, chills, tachycardia, hypertension, 101.4, swelling and redness around the site, headache, diarrhea. Headache, diarrhea, swelling and redness have not gone away. Went to ER after reaction started, was evaluated in ER. Physician stated it was typical vaccine side effects, no treatment or epi was needed.

On 12/31/2020 I had a headache and I was tired. Over then next two days my arm was very sore. That subsided by 1/02/21. On 1/07/21 I woke up around 8 AM with my arm being sore again and at ejection site I had a red circle. The ejection site was also swollen. I took a benedryl and the pain subsided but the red circle continued to grow. On 1/08/21 the circle got even bigger (even with taking two 25 mg tablets of benedryl before bed) and it?s warm to the touch. Also on 1/07/2021 at around 6 pm I began menstrual spotting (this would be in the dead middle of my usual cycle). It continued into today ,1/08/2021. This is unusual for me as I have never experienced spotting in the middle of my cycle. Also on today 1/08/2021 my arm soreness has returned as the red circle is growing bigger around the ejection site (approximately 3 1/2 inches by 2 inches). I?m also experiencing extra tiredness and a headache.

After shot, I had swelling of lips, hives on neck and arms, and red marks on hands. This was about 5 minutes after shot. Next day I had chills, nausea, headache, sore throat, and shakes. Started the afternoon of the next day. My arm has been red and itchy since the day, but it is increasing becoming warmed, bigger, and itchier every day since.

15 minutes after vaccine- hot all over body, throat closing, itchy, eyes felt weird, dizzy, and rapid heart rate

I developed a bumpy itchy skin rash at the injection site 1 week after my vaccine. The bumps are itchy and are only at the injection site (upper left arm)

On Friday, January 8 I experienced a headache for the remainder of the day following my vaccination. This headache went away when I woke up the following day. On Saturday, January 9 I experienced chills, nausea, body aches, and a stiff neck, along with pain in my left arm. The injection site was inflamed and painful. Most of these symptoms were relieved the following day. On Sunday, January 10 I experienced pain in my left arm that has travelled into my axillary area and my left shoulder. This has yet to be relieved. I am also erythemic at the injection site.

At around 11 pm, I started to feel nauseous. I had had a sore arm all day which I expected, but after trying to sleep it off I woke up around 2am shivering and sweating through my shirt at the same time, and had a 98.7 temperature (normally I am 97.2 so that's high for me). My arm started to have some swelling at the site, but I woke up this morning and the fever, chills, and nausea are still very much present about 24 hours after receiving the first dose.

Pt reports dizziness, slurred speech, shakiness, and red/inflamed upper extremities (finger-tips) 3 hours after vaccination. These symptoms resolved with the use of Naproxen after 2 hours (by 5 hours post-vaccination). Pt also reports that she experienced full body aches and fatigue that started 3 hours post vaccination and lasted for 1 full day. Pt also reports that she experienced headache that started 3 hours post vaccination and lasted 3 full days (but she does suffer from chronic headaches).

On 1/5/21 Patient noted that the deltoid in which she received the vaccine was swollen, with bluish looking marks initially that transitioned to redness and a patchy, itchy, raised, red rash that spread down her arm, across her chest and abdomen. On 1/6/21, the rash worsened, resulting in patient going to the ER. Benadryl was prescribed and she was told to follow up with her primary MD.

Pt reported that at about 7pm the day of her vaccine she developed a rash on her left arm/leg. It was itchy and so she took a Benadryl. She said her "throat felt thicker" but the Benadryl helped. She went to sleep and was fine. When she woke up the next morning on 1-6-2021 she reported the rash had spread. When she got to work and found me about 9:30 am to ask if that was normal I encouraged her to call her PCP and take a Benadryl. At that time she was having no difficulty breathing. She chose to call her PCP and not take a Benadryl as it makes her sleepy. After she called her PCP she ended up leaving work. I found out later that her breathing had become labored and her neck/throat was starting to swell. Per her report, her PCP gave her epi and steroids, and R/X her an epi pen. She took her medicine and Benadryl around the clock and returned to work on 1-8-2021. When I spoke to her on that day she stated all of her symptoms had resolved, but her lymph nodes in her neck were still a little swollen.

pt states that she had vaccine on 1/9 later that night she had splochy rash on chest that resolved on own over night sunday 1/10 she felt feverish, bodyaches and chills with nausea and vomiting continued all day and over night was taking tylenol with mild relief of fever and body aches syptoms have somewhat resolved

Patient received Moderna COVID 19 at COVID Vaccine outreach. Prior to administering vaccine patient was screened for any severe reactions to a vaccine or allergies. After patient was administered vaccine by MA at 1025 am and as waiting out her fifteen minutes post vaccination and began to 'feeling funky' according to patient. States that this is what happens when she has had a reaction in the past and then progressed to coughing and itching without hives.Patient did have flushing and redness of skin. RN attended to patient and her heart was in the 70's. RN gave 50mg IM Benadryl and EMS was called. EMS arrived and patient was checked out and cleared to leave. RN's were there to help pot benadryl administration.

1/11/2021- noticed red erythematous, itchy severely rash to Left forearm. Improved with otc antihistamines 1/11/2021- received 3 additional vaccines from PCP - MMR, Tdap and HPV- later that same evening rash developed to left arm where one vaccine provided our of 3. Resolved by 1/12/2021.

Severe headaches, fatigue, fever and chills, moderate to severe and currently ongoing swelling, redness, itching, pain at injection site with the site continually getting larger, altered sense of smell, coughing, shortness of breath, moderate to severe heartburn, gi upset and diarrhea, altered taste. All currently ongoing

The first two days after injection it was painful to lift arm above 30 degrees and hurt upon palpation and I had an overall tiredness. A week after the injection on 1/13/2021 I developed a raised, red rash surrounding the injection site (2in x 4 in). The skin that was inflamed was 101.2 degrees fahrenheit. The intense rash lasted around 24 hours and subsided after 3 treatments of Benadryl cream and one dose of oral Benadryl. The skin is still slightly raised and has a slight redness to it today (1/15/2021) but is much better than before.

She reports that the injection site was red and swollen for a day or two after the vaccine then resolved. Then 6-7days post vaccine the injection site has become red, swollen, sore and itchy. She also has body aches at this time.

She reports that the first 2 days after her vaccine her arm was red, swollen and sore then resolved. She now reports that starting around 6-7 days after the vaccine she developed redness, itchy, swelling and soreness at the site. She also has body aches at this time

Pt stated that bruising started almost right off on 1/6/21 but itchiness redness and swelling started approximately a week later on 1/13/21.

The following day I experienced headache, nausea, fever/chills, and muscle aches. It lasted about 24 hours. I also experienced injection sight reaction. It got red and swollen, hard and painful to the touch.

Pt noticed itchiness and small bumps covering both forearms in early AM on 1/15, arms proceeded to worsen in redness, slight swelling with bumpiness around the edges, pt took 20 mg of prednisone around noon on 1/15. Pt woke up on 1/16 with rash now in right inner thigh, palm sized, as morning progressed pt noticed rash on top of butt/ lower back, extending up into the armpits on both arms, and on feet. Pt was in a lot of pain, pt took a hot bath, 40 mg of prednisone and 25 mg Benadryl. Pt took 50 mg of Benadryl night of 1/16. Pt awoke on 1/17 with worsened symptoms, pt repeated doses from 1/16.

After one week of getting the vaccine my left arm, right around where I got the injection, formed a rash and became very red, swollen, and hot. After taking Benadryl and putting on hydrocortisone there has been no changes. It itches only if something rubs against it or if i touch it. It is about 3inches in length and about 3inches in width.

The second day, headache and nausea but went away. A knot that went away also. Has a hive that's red and hot to the touch.

telephone call -itchy arm, neck, ED visit, RX benadryl, released after 1 hr, no resp/thrt, allergy consult; Incident rpt: - I had received the vaccine around 4:45pm and was there for the 15 min of observation then went off to the floor I was floated to. Next around 6.40-6:45pm I started feeling chills and felt extremely itchy on my arm (the one I received the vaccine) my neck, and my eyes felt so itchy and puffed up. Then, the charge nurse sent me down to the ED because I was experiencing an allergic reaction; is advised to get second vaccine in allergist office

Received vaccine on 1/12/21. On 1/19/21, I woke up to a red splotchy rash on my left arm where I received the vaccine. I put cortisone cream on it, and that night I noticed it is now raised and more red. It does not itch or hurt.

metallic taste in mouth immediately following vaccination resolved by evening warm swollen red spot at injection site fatigue resolved following day

Received Moderna COVID vaccine #1 at 1207. Experienced tingling, chills BUE 7 minutes after vax admin. Then seeing black dots, head felt heavy, nausea, tingling now BLE as well as BUE, and rash beginning to anterior chest. Did not give diphenhydramine in vaccine clinic because patient nauseated. ED Charge nurse called and patient transported to ED for treatment. She received IV diphenhydramine and experienced vertigo, then given 1 dose of meclizine and IV fluids. Symptoms resolved, including rash. Discharged from ED at 1718.

Moderate pain at injection site for over 48 hours Vomiting, chills, shakes, sweats the whole night after the vaccine. Moderate Inguinal pain began on 2nd day after vaccine. Full body aches began a few hours after the vaccine and continue on. Moderate chest pain and tightness ongoing since the day of the vaccine. Severe decreased appetite ongoing since vaccine. Severe fatigue since vaccine and continues.

pt reports she currently has 101.2 fever, headache, body aches, fatigue, and left arm (under arm and across chest ) is sore and injection site is sore and swollen.

Two days after receiving the vaccine I had a sore throatwith white patches and fever nausea I went to the doctor and tested negative for strep flu and covid and said I had an unknown bacterial infection and prescribed me clindamycin and after 3 days of the antibiotics I started feeling better

01/12/21: Severe fatigue and brain fog 01/17/21: Pharyngitis of unknown cause. Fever over 102F, bilaterally swollen tonsils with white coating, bilaterally swollen lymph nodes in the neck, headache, fatigue. Initially prescribed Penicillin 500mg TID x 7 days (Telehealth). After day 3, symptoms continued to get worse. Strep test, Flu test, and COVID-19 tests were all negative. Prescribed Prednisone 40mg x 7 days. Symptoms began to subside on 01/21/21. Cause still unknown - possible viral infection (CMV, EBV). 01/21/21: Swollen, red rash began to develop at injection site. Subsided without treatment within 24 hours.

No adverse events until 5 days after. Site is now red, hot to touch, swollen, and itchy and seems to be getting worse.

2:00PM 01/22/2021 Neck is numb from right side; 8:00PM 01/22/2021 Right arm was sore, couldn't move it. Had a headache; 02:00AM 01/23/2021 Entire body was sore; 12:00PM 01/23/2021 Entire body was still sore, right arm hurt a lot. High temperature; 3:00PM-Currently 01/23/2020 High temperature, chills, sore body. Feeling lethargic and cannot get out of bed because of the pain.

Painful, red and white, blister like bumps all over the hands and feet. Began as a single bump (01/22/2021) then spread throughout both hands and feet the next day (01/23/2021). No other symptoms. (I had coxsackievirus as a young child)

the injection site is still hard, red, and hot (larger than a quarter) 9 days after injection. The arm is still tender to the touch (1 on a 1-10 pain scale). The inflamed area is slowly growing. No treatment applied.

- Injection site pain lasting for four days, on first two days pain was severe, on the third day the pain was moderate, and on the fourth day was mild. - Body weakness/fatigue for the first two days. - Chills with diaphoresis on first two days. - Elevated body temperature, but no fever on first two days.

The patient started getting itchy on her face and neck 40 minutes after receiving the vaccination. Soon after in the evening the patient had rash and hives on her upper torso and neck. The hives spread the next day to eventually cover her entire body. The pt. took a Benadryl and went to the hospital because of the severity of the hives and itching on day 2. The staff told her that she should use hydrocortisone cream and should possibly not get the second dose of the vaccination. The pt. continued to use both the Benadryl and hydrocortisone the following day, but still the hives persisted. Pt. is on Day 4 and will let us know further details on her condition as time goes on.

I received the Moderna vaccine at 3:50pm on 01/25/2021. Around 3:30am on 01/26/2021 I awoke feeling feverish and nauseous, and my arm at the site of injection was extremely painful. For the next 4 hours, I drifted in and out of sleep as I experienced violent body shakes from chills. Around 7:00am I woke up feeling more nauseous, and my temperature read 99.7 F. I vomited once, just containing stomach acid. After vomiting the nausea subsided but I had a pounding headache with sensitivity to light, joint and muscle aches, and continued pain at the site of the injection, although no redness or swelling was noticeable. The headache and injection site pain improved a little bit with Tylenol. At 11:20am 01/26/2021, I am now experiencing a mild headache and body aches, and my temperature reads 99.1 F.

RED, ITCHY RASH AROUND AND BELOW INJECTION SITE. DECREASED BLOOD PRESSURE REPORTED BY PATIENT'S MOM TO BE 105/66.

A little over a week after receiving the vaccine I have developed a rash around the injection site. It is a circular rash that is slightly raised, mildly itchy, and warm. I am not experiencing pain.

The night following the vaccine, I experienced a fever above 100 degrees, with moderate chills, body aches, and joint pain. The injection site was swollen and red in addition to general muscle pain from the shot. This continued until about 48 hours after the vaccination.

Started to feel sx on 1/24 with nausea, fatigue, feeling of warmth, dry mouth, and her face and eyes were swollen. The injection site was red, hot, throbbing and the size of a tennis ball. On 1/25 she was congested, had a headache and the injection site was red, hot, sore to the touch and sensitive. Most likely is an allergic reaction with facial swelling and local reaction, treated with Benadryl Motrin and add Pepcid or Zyrtec. On 1/27 she was better with no fever x > 24 hours and less fatigue. No body aches and no trouble breathing.

Threw lot# away. 1/3/21 am got hives on ankles, 3am, Then around hands 7am. Then Hives around hips. Then went to the back. 1/4/21 Dizziness when trying to get up. Symptoms lasted 1/5-1/6. Went to urgent care because hives were all over. Rec'd Medrol from urgent care along with hydroxyzine. Got relief on 1/9/21.

Maculopapular rash all over area where vaccine was given and has spread across chest, upper back, neck and other shoulder. Not painful, mildly itchy.

Probably about ten seconds after receiving my second dose of the vaccine I stood up and walked out of the room and all of a sudden I felt my face get very hot and go numb and tingly and then my I started having trouble breathing. I went back into the room and still couldn't breathe so they sat me down and told me my whole body was bright red and they stuck me with an epipen. The whole reaction probably lasted a little less than a minute from the time it happened to when they stuck me with the epipen.

About 3-4 hours after the injection was given, my left arm began to hurt at the sight of the injection. It was sore red and beginning to swell. After a couple more hours it was difficult to even move my arm; it felt as if my tricep was almost entirely immobilized. These symptoms persisted for about three days, with increasingly less intensity, and after the fourth day, I had no adverse symptoms. Exactly six days after receiving the vaccine, the area where the injection was given (tricep) began to feel itchy. When I tried to scratch it, the itch was not relieved but only worsened and began to swell and turn red. By the following day (7 days after injection), the site of injection and area surrounding the site of injection (about 6 inches along my upper and 3-4 inches in width) had swelled up more and was significantly more inflamed than my opposite arm which hadn't changed at all. It had a similar swelling, itch, and pain to the sensation one experiences after getting bitten by a mosquito, but in this case a very very large mosquito bite. By the evening of the seventh day, I began feeling very fatigued both mentally and physically and felt very subtle body aches that usually signal the early signs of a fever (at least for me). Upon feeling feverish symptoms I made a drink of turmeric and ginger root powder with honey and laid down for about an hour in an attempt to rest. After an hour, the body aches and fatigue lessened in intensity but my arm was still swelling and now feeling hot in temperature. The itching was unbearable. I went to go take a warm shower later that night and started to feel a mixture of pressure and pain traveling up my arm and into my chest. I began feeling my lymph nodes on the left side of my breast to find that a lymph node in my armpit and another one on the side of my breast had enlarged to about two inches in diameter. They were painful to the touch and noticeably sore when left untouched. These symptoms persisted with no change for about 2 days. After two days, the swelling and itching in my arm went away but my lymph nodes were still inflamed. They did not reduce in size or in pain until about 7 days after the first signs of these symptoms. After a week all symptoms went away.

Itching accompanied by small raised red dots on skin, mostly on hands and arms, bilateral, not near injection site. Minor discomfort, treatment not initiated.

After one day of vaccaination, the area near the area of vaccainated begin to swell and pain a lot especially for the whole muscles in the arm. It is still a little bit itch in the skin. After one week of vaccination, the skin of the same area begins to appear small red blocks and begins more itch in the skin.

Patient presented for second vaccine. Approximately 10 minutes after administration, rash appeared on right arm and right side of her back. Pt states that she has had this happen before but feels that it might be "in her head" as well. Pt has significant list of medication allergies, none present in Moderna COVID vaccine. Pt continually monitored, rash continues to spread. Decision made to transport patient to hospital by ambulance. Pt begins to report that he lips are numb and that she feels her throat is closing. Pt transported by Ambulance to hospital.

Patient reported tingling in her feet bilaterally about 15 minutes after vaccine administration. Then noticed hives on shoulders and chest bilaterally. A rapid response was called and patient was found to be hypertensive (160s systolic) and tachycardic.

The symptoms began immediately after administration of the vaccine. The symptoms were: tingling of her back / spine and then two hours later had itching swelling of the throat like she couldn't swallow felt lightheaded and dizzy and took benadryl with improvement. On day 2 felt feverish with sore arm and then on the 3rd day developed the same symptoms as the first day and went to the ED after taking benadryl x2 with no improvement in her symptoms. Objectively there is no wheezing, swelling, or rash but the provider does note that there were 3 small excoriated spots on her wrist one of which looked like a hive. The treatment of the reaction was: o Antihistamines: benadryl 50mg at home and ibuprofen o Steroids: IV solumedrol o Epinephrine: x1 o ED admission

Red spot appeared on left arm in shot area two days after first dose of vaccine. The redness began to fade but has returned. With the recommendation of a nurse friend, I drew a line around the affected area and the redness has grown past the line. The size of the redness is about 3.5 inches wide and 1.5 inches tall. The area is also very itchy. It is concerning because the redness had almost dissipated and now has come back over a week after the first dose.

Formation of medium-sized, raised, warm, red patch at site of injection . Noticed it on 02/10, not sure how long it has been present for. I have had a headache today as well (02/10). Not sure if it has anything to do with the rxn

developed diffuse blistering and burning rash to face along with fatigue 4 hours after receiving. 2/6/2021 had an episode of dizziness and vertigo

Initially there was only pain at the site and fatigue. Later in the evening it became chills, fever, body aches, headache, loss of appetite, nausea, vomiting, fatigue.

About an hour after administration, she started feeling like she couldn't catch her breath. She also stated that it felt like her tongue was too big for her mouth. She also stated that it looked like there was a mosquito bite at the site of administration. She took a benadryl (unknown strength) about 1 hour after developing the breathing symptoms, and an hour later, she was starting to feel better. As of 24 hours after, the administration site reaction is not resolved.

Headache, Myalgia & NauseaVomiting Narrative: dose 2 ha- ha;chills;n/v;fatigue;pain and redness at inj site

After 8 days of my shot (today), I noticed I was pretty itchy on my arm at the injection site. When I finally got a good look at it, it was red and had, what seems to be, little hives around it. It?s also a little tiny bit swollen at that area. Very warm to the touch and I can now feel this continuous, residual heat around it now that it?s later on in the day. Since it?s only the first day I noticed it, the size of the area is still rather small. I don?t know if it will progress to be any bigger than it is now or if its appearance will change in the future.

COVID dose #2 given on 10FEB @1330. Notes about 1 hour after began to get HA, chills, rhinorrhea, nasal congestion, sore throat, cough, nausea, fatigue and body aches. Shortly thereafter broke out in diffuse red itchy rash of the body. Presented to the ER and was sent to FM to make appt in respiratory clinic. She denies ever having difficulty breathing, sob, wheezing or increased work of breathing. She took 2 Benadryl (50mg) and the rash resolved. Woke 11FEB with L arm tingling and L upper lip swelling. L arm tingling has resolved however lip swelling worsened throughout the day. Denies swelling of the inside of the mouth, tongue, itching or swelling in the back of the throat. Denies globus or difficulty swallowing. Denies dyspnea, wheeze or change in voice tone.

Moderna COVID-19 Vaccine EUA I received the vaccination around 2:45 pm and felt mild soreness at the injection site within a few minutes. At around 4:00 pm, I felt mild joint pain in my right ankle which worsened to mild/moderate joint pain in my left elbow/wrist + both ankles/knees, so I took an extra strength acetaminophen (500 mg) around 4:30 pm. My joint pain subsided, and I began to feel fatigued around 5:00 pm. Mild chills began around 9:30 pm, and joint pain at the same sites resumed around 10:00 pm. I went to bed around 12:00 am with moderate chills, mild joint pain, and mild pain at the injection site, but I awoke around 12:45 am with slightly worsened chills, joint pain, and pain at the injection site. I took another extra strength acetaminophen and was able to sleep for the rest of the night. I woke up at 9:00 am with no symptoms.

Developed a serious bruise at injection site overnight. Bruise was accompanied by severe arm muscle pain.

After the shot, my arm was swollen for 3 days and a small rash but it went away. Then, on Feb 13th a rash started to reappear that was warm to the touch and itchy and burning. I applied cortisone but rash on injection site kept growing. On Feb 14th, I woke up and rash was a bit smaller but every two hours I would have a tingling sensation like when your foot falls asleep but it went up my shoulder and back area up to my neck and occurred every 2 hours. I then was advised to take claratin, add more cortisone, and take Benadryl as well. The itchiness stoped. My rash is still there but it?s very light compared to feb 13th.

At 2:00 am on 2/13/21 I developed fever, chills, body aches that continued till 2/14/21. Then on 2/14/21 my body broke out in an itchy rash on my neck and my wrists/lower forearm. Then on 2/15/21 my mouth formed ulcerations.

A dull ache in injected arm 1 week after injection and continuing on to current day (over month from injection) the ache extends from the left deltoid, to the shoulder, under the armpit and feels like it radiates to left breast but really unsure. Hard to describe the feeling and exact location.

Onset of fever of 101-102 degrees Fahrenheit approximately 45 minutes post administration; poorly controlled with oral acetaminophen. Nausea was also present, but subsided ~5 hours post-administration. Arthralgia, myalgia, and migraine headache severely limiting activities of daily living which were not present prior to administration sustained at time of report. Suspected SIRVA to be evaluated by PCP in one week.

The day after the second dose, a small red bump first appeared on the stomach. 8:10 PM to spread all over the trunk of the body and spreads every day little by little covering the whole body. Bumps are very itchy.

Large spreading lump that went down after three days, started the day after the vaccine, 8 days following the vaccine injection site has puffed up and is itchy and painful

This came 2 weeks after that i take the first dose of the vaccine, its like a rash, dryness, and red, at the beggining was 15, and know after the second dose of the vaccine more of this kind of rash appear.

Hives all over my face and ears, onset 7 days post-vaccination, on and off for four days so far, on different parts of the face

Patient passed out just after getting to work. She hit her head - cheek & chin - causing abrasion/contusion. This was NOT wittnessed; she does NOT recall passing out or hitting her head. A co-worker noticed something wasn't right & had her sit down. They called my wife who picked her up & brought home & rested. Approximately 5.5 hours later, after feeling "better," she got up to get something to eat. While returning to bed (to rest & eat the small container of applesause) she felt as though she were going to pass out again & quickly sat down on the steps. However, she did not call for me (I was in the room nextdoor) for a few minutes... seemingly having "zoned out" but she does not think she lost consciousness.

Nickel sized bruise appeared after vaccination and has not gone away yet. Flu-like symptoms began, waking patient at 0300 hours on 2/18 in a state of confusion. noted ringing ears, dizziness, abdominal distress that resulted in diarrhea, as well a headache and backache. After struggling to get up and failing the first time, patient was able to relieve self in bathroom before going back to sleep until about noon. Patient took ibuprofen before going back to sleep. Flu like symptoms persisted into the next day, and included feverishness and fatigue. Patient was feeling better by 2/19.

On Day 7, I noticed a large swollen lump where the vaccine was injected. It was red, itchy, and hot. Most of the swelling went down after 24 hours, but 4 days later I still have a very large rash that extends from my shoulder to my elbow. It is red and has clear borders. It is still itchy and slightly warm.

On day 8 after receiving the vaccine there was a red circle around my injection site along with redness, tenderness, a hive on injection site, and itchiness. No medications. Resolved in 3 days.

Big bump, red and hard at injection sight. Hurts to touch. Warm to touch. Appeared following morning. Slight fever and fatigue as well.

received vaccine on 2/24/21 at 530pm. this was her second dose and had no issues with first dose. on 2/25/21 around 12pm she became woozy and then around 5:15pm began seeing black spots. She didn't eat dinner and went to get in shower. At injection site she was red and warm to touch. She took Benadryl 50mg and then later began having difficulty swallowing. She went to local ED at 9pm. At the ED they gave her a normal saline bolus zofran, solumedrol, and rocephin. She fell asleep at the ED and after 1 hour her swallowing improved. On 2/26 her arm was still sore with redness and swelling at injection site. no hives or weakness noted. She went home on prednisone and antibiotic.

Student reported to Campus health services on Friday, February 26th after receiving the COVID vaccination on the prior Wednesday. Student reported development of symptoms the Thursday after vaccination. Student presented with redness, firmness, induration, pain and warmth to injection site. Induration measured at 47mm diameter. Student reported back Monday, March 1st. Redness resolved. Still firmness at approx 50 mm in diameter. So pain, but improvement.

WILL OBSERVE. 1055: BP 118/64 HR 98 RESP 14 O2 SAT 99. MILD REDNESS AND TENDERNESS TO PALP AROUND INJECTION SITE. PT STATES THEY FEEL NORMAL.

I have a round, hard, itchy spot on my arm where the shot was administered. It isn?t too bad but definitely noticeable. I first noticed it last night. Also I am 41, not 21, but for some reason the form insisted I put 21.

Some of the symptoms that were felt after 3 minutes of getting the Moderna vaccine, I felt dizziness. I couldn't see anything and I felt a little nauseous in the stomach. I was able to lay down for a couple of minutes and felt fine afterwards. Some of the symptoms going home are a sore arm, can't seem to lift it up, tired, the vaccine site seems to sting, and some chills.

PT WAS GIVEN VACCINE ON 2/27/2021 AND BEGAN HAVING SYMPTOMS ON 3/5/2021. PT ARM BEGAN HURTING AGAIN , ARM WAS RED WITH A KNOT AND WAS WARM TO TOUCH. ALSO HAVING PAIN THAT GOES DOWN ARM. SYMPTOMS ARE SOMEWHAT BETTER TODAY.

3/9 (Tues): Sore arm, red 3/10 (Wed): Sore arm, increased redness; Wed p.m. - chills, nausea 3/11 (Thurs): increased redness, chills in evening 3/12 (Fri): increased redness with area (12cm x 8cm). Treated with cephalexin, cellulitis Will recheck 3/15; allergy eval prior to next dose

Patient received Moderan dose 1 vaccine and felt lightheaded, presyncopal and then reported numbness and tingling to the shoulder and trapezius muscle on the left, ipsilateral to the injection site. She felt better after 30 minutes and proceeded to go eat in her bedroom. She was nervous and anxious prior to the vaccine and states that she thought that had something to do with it. She then experienced a 10 second bout of muscle tension in her face and jaw which caused her to be unable to talk or speak for a few seconds. She was brought back down to the hall for evaluation. She felt as if she was having a stroke and that got her more worked up. She denies lower body weakness. She states that her head feels heavy and she complains of general fatigue. No n/v. No headache or blurry vision.

Sight of injection started swelling, getting red, and getting hot about a day later. Redness has progressively grown in size over the past two days and it has continued to be swollen and hot to the touch. It?s now about 2 x 3 inch blotch and has started itching in the past 24 hours. I did not have any reaction to the first vaccine except a very sore arm.

Intense hives that developed over 48 hours into a rash of hundreds of hives on bottom of feet, tops of feet, ankles, back of calves, inner thighs, and under arms near biceps. Hives were symmetrical in nature and mirrored a ?covid rash? or urticaria. Swelling intensified even while on Benadryl. Only steroid began to help diminish redness after 24 hours on 80mg of prednisone. Began 3/10, worsened exponentially until taking prednisone on 3/13. Now (3/14) healing but still experiencing itchiness and red bumps.

Slight headache and sore arm at site of injection three hours after the shot (3/12). The morning after, about 16 hours after the shot, I had severe body aches and pains, a severe headache, fever, chills, and was extremely fatigued with worse pain in the arm at the site of injection (3/13). This lasted until I woke up the next day, about 40 hours after the shot, I felt physically weak the next day and still had arm soreness at the site of injection but no pain, headache, fever, or chills (3/14). Felt completely back to normal, still some slight pain on the arm at the site of injection, about 64 hours after the shot (3/15).

The day of the shot I experienced a moderate headache throughout the evening as well as soreness at the injection site. The day after I received the shot I experienced severe muscle fatigue and a red rash, bruising, and swelling around the injection site.

Patient received dose of Moderna Covid vaccine at 8:07 am and at 8:15 am she begin to complain of throat feeling "hot and tingling". She was assisted to an exam room to be monitored. B/P 140/80, pulse 74, O2 sat 99%. Skin cool to touch but not clammy, coherent in speech, no shortness of breath noted. She requested water to drink and stated her throat felt better. VS checked every 15 minutes and were normal. No SOB through out visit. At 8:55 am VS were: b/p 120/70, pulse 71, O2 sat 99%. Patient escorted to car at the end of monitoring for 55 minutes. She states she was afraid as others she worked with had a lot of side effects with the second dose. No meds given as it appeared patient was anxious and not having an anaphlylactic reaction. Phone call to patient at 11;30 am and she stated she feels better only soreness to arm where vaccine was given.

The morning after the vaccine, I had a really awful headache and swollen arm, that was incredibly touch-sensitive. Around 3 pm that day, 25 hours after the injection I was hit with severe nausea. About an hour later, I began vomiting and had diarrhea. For some time, I was unable to keep water down. I called a 24-hour nurse line, and they advised contacting my doctor's emergency line. I did, they said if I couldn't make it through the night without keeping water down I needed to go to the hospital. Luckily, right before bed I drank some water and was able to make it through the night. The next morning, I had a telehealth visit with someone from my doctors office to review symptoms. Today I have had waves of nausea, but no vomiting. I have had diarrhea, and I had a spontaneous period that was very heavy and two weeks early today (2 days post vaccine).

Large red rash on injection site, swollen, warm to the touch, hardness, itchiness, discomfort/pain when moving arm or laying on it - all around the area of injection site - started on March 14 and has continued with the same effects listed though it has become more red since then- treatments I've tried are icing the area (throughout the day)and antihistamines (at night only) provides temporary relief but symptoms comeback

immediately felt sore on site of injection and within minutes of leaving clinic, i felt sharp pain in my upper left back (still have that discomfort and I looked at my back this a.m. and i have a red bruise like mark) I took advil and laid down, fell asleep and when I tried waking up, I couldn't wake up and when I did wake, I was breathing hard and disoriented. I slept with my mom in fear of having episode again. Around 2:45 am, my mom woke to me shaking like I was having a seizure. it lasted about 30 secs. she called 911 and i was checked out by paramedics, who said my vitals were fine and I should flollow up with medical care. My mom and I agreed that perhaps I needed stress dosing of Cortef. I took stress dosage and have been taking stress dosage all day and so far no more episodes. Now waiting for Doctor to call me back and advise.

On 28 Jan 2021, prior to the onset of the symptoms, the patient received their first of two planned doses of mRNA-1273 (Lot number 031L20A) intramuscularly for prophylaxis of COVID-19 infection. On 28 Jan 2021, the patient experienced fainting, shaking, cold arm, red spots on forehead, and chills. Reporter stated that no treatment drugs were prescribed for the events. A complete blood count and an electrocardiograph was performed (results not provided). On 29 Jan 2021, the patient experienced fatigue. Action taken with second dose of mRNA-1273 in response to the events was not provided. Hives on arms and chest. Swelling and difficult breathing. 24 hours after injection. Headache, fatigue, flu like symptoms.

I had a bad headache begin a couple hours after receiving the vaccine, and muscle soreness. I woke up and could barely move my arm, and throughout the day I got progressively more fatigued. By the next night, I had a low-grade fever and had extreme all-over body soreness. It felt like I had the flu. By morning, I was feeling a bit better and got progressively better. The next day (2.5 days after the vaccine), I was feeling better other than some soreness in my injection site.

Minutes after receiving the first dose of the Moderna vaccine, patient complained of difficulty breathing. I asked if she was wheezing and she said no. A doctor in the clinic came over and even though it wasn't time for patient and her sister to leave (based on 15-minutes after receiving the vaccine) he initially suggested that they might be fine to go, but after patient told him of her symptoms and at my urging he decided to have her stay a little longer. I think it was at this time that he had someone check her pulse. I don't know what the pulse reading was. He stepped away and shortly afterwards patient started complaining that her throat felt strange and was making it hard to breathe. The doctor immediately came back over, found patient was having difficulty breathing and holding herself upright in the chair. I held her by the shoulders to keep her from falling while the doctor also steadied her and asked a worker in the room for an epi-pen. The worker didn't seem to react much to the request so he told her louder that he needed an epi-pen. She started searching her area and the doctor started yelling across the room to others that he needed an epi-pen. It seemed like others initially took the demand less urgently than they should, but eventually someone came with an epi-pen. By this time I don't think patient was very conscious, if at all. Sometime during this time the doctor also instructed someone to call 911. The doctor tried using the epi-pen but at first he had some kind of cap on it that prevented it from working. He removed the cap and not finding an easy way to have a sterilized injection site he injected the epi-pen through patient's jeans. Soon afterwards a cot was brought out and workers had me step aside as they took patient and put her on the cot. Patient seemed to gradually become more aware of her surroundings and it wasn't long before she appeared to me to be fully conscious once again. They checked her blood pressure and pulse and maybe did other tests during this time. Eventually the ambulance arrived and patient was very clear with everyone that she did not want to incur the expenses of riding in the ambulance and I offered to bring her wherever they suggested that she should go. They watched and evaluated her for a while and eventually had her sign the form declining the ambulance transportation. They suggested that she make an appointment with her doctor but did not suggest that we take patient to an emergency room. We departed and patient did not afterward seem to experience any symptoms from the vaccine other than some of the general feelings of being sick, soreness at the injection site, etc. that others report with the Moderna vaccine.

Reaction started out with a localized 3-4 inch inflamed reaction. It looked very similar to hives. This was on Friday, March 19th, 2021 around 9 PM. I went to sleep and woke up the next morning and it was still there. I took benadryl and one ibuprofen to help the inflammation and the reaction, but it only got worse following this. I then had intense tactile and mechanical allodynia, which is often associated with my diagnosis of fibromyalgia, but this has been very intense. The allodynia had been occurring across my left hand up to my elbow, but has now moved to both of my arms, my neck, my entire back (more severely around the ribs), my chest, and my feet. I am very fatigued and feel feverish. My current temperature is only 99.2, but the pain is still active. Today is Sunday, March 21st, 2021. I got the vaccine on Saturday, March 13th, 2021 and this reaction is following the vaccine about a week after.

The syringe was larger and looked full. It took a long time for medication to go into the arm due to the amount of liquid injected into the muscle. Right away the full left arm and breast were painful. The arm pain continued to get worse and felt like she hit her funny bone but more extreme and this pain lasted all night into the next day. She was unable to sleep due to the full arm pain. The significant pain in the breast area lasted all day into the next day too. Also, felt chills and body aches starting that evening into the next day and night. We did not know the dose was only supposed to be 0.5 ml at the time of injection. We suspect she received at least 5 ml instead of the recommended 0.5ml.The name of the nurse that administered the vaccine is written on the vaccine card but not sure of spelling. RN?

on 3/19/21 I developed covid arm and the swelling, redness. irittation, and itching has progressed since then.

The patient's medical history was reported as migraines. Concomitant medications were not reported. On 27 Feb 2021, prior to the onset of the event, the patient received their first of two planned doses of mRNA-1273 (Lot number: 013A21A) intramuscularly in the right thigh for prophylaxis of COVID-19 infection. On 27 FEB 2021, the patient experienced Raised hive (Urticaria) about the size of a half dollar, after that she "felt off" (fall) had a headache (Headache) and Felt crappy (Malaise), and she had a couple of bad breakthrough bleeding episodes (Hemorrhage) which was assessed as medically significant, and the patient also experienced interrupted Menstrual cycle (Menstrual disorder). On 07 MAR 2021, the patient experienced Turning into a bruise (Contusion) and on 08 MAR 2021 it was weird again (Feeling abnormal). Laboratory details were not provided. Treatment medication was provided as Tylenol. Action taken with mRNA-1273 in response to the events was not reported. The morning after I received the Moderna vaccine I saw a bruise on my right rib cage. The shot was given on my left arm.

Site: Itching at Injection Site-Mild, Systemic: Dizziness / Lightheadness-Mild, Systemic: Fainting / Unresponsive-Mild, Additional Details: Patient discussed when going over the list she has syncope that occurs randomly. In my better judgment patient looked normal and healthy at taht time. Patient 5 min later felt dizzy and fainted for 10 sec before recovering. I called 911 and gave water while the other rph gave cold compress. Patinet recovered quickly when ambulance came but patient refused further medical care and has had history when given vaccine this dizziness occurs. Family member came to pick her up and walked without issue

Adverse Event: The patient felt throat itching after 10 minutes of covid 19 vaccination ( 11:47 AM), thereafter the patient complaint throat closing. EMS was called @ ( 11:49 AM) Treatment/Intervention: The patient was administered epinephrine 0.3ml on the right lateral thigh and EMS was also called (11:55 AM). The patient complained of itching and Benadryl 0.5ml was administered on the right deltoid. Outcome: Throat constriction subsided and the patient only reported scratchy throat thereafter. EMS arrived on-site @ 12:10 pm and provided oxygen via nasal cannula @ 15 L/ min and then transferred the patient to a local hospital

Large, tender, and erythematous rash present on the left shoulder (immunization site), which started on 3/19. Rash is 12 x 12 cm in size, Pt without other symptoms to include fever, chills, cough, sob, chest pain, seizure, or other neurologic symptoms.

First day I had immediate swelling at the injection site, red circle form around the site of injection following up with an itch that lasted for 3 days. Along on those immediate symptoms I got the injection on my right arm and I experienced pain shooting up from my right arm into my neck and currently still have those pains from time to time when I move my right arm at an elevated angle. Day 4 of the vaccine I woke up one morning with swollen glands, congestion, swollen nasal glands, low grade fever of 100 degree F, very itchy irritated throat and hard to swallow, visited a clinic to see if I possibly had flu or strep but all my test results returned negative, also I had a covid test done and it was negative, I have never contracted covid 19 prior to vaccine and current day. Additionally, symptoms of congestion worsened and I am now experiencing some laryngitis due to the swelling of my lymph nodes, only sinus medication lessens the congestion to manage, I feel very fatigue, and experiencing some migraines. Today only marks 1 weeks since the vaccine

Rash at injection site eight days after shot. Mild itching. Taken antihistamine and using topical steroid to treat.

Within 30 min of the shot, my arm began to swell and itch. My face and some part of my body began to itch as well. Within the next hour, my jaw began to lock and my throat began to burn/tighten. Within this timespan, my body, especially my back began to ache as well as my chest feeling tight. After three hours, I went to the on-campus clinic and they gave me 50mg of Benadryl.

I received an allergy immunotherapy shot the day before my first dose of the vaccine and instantly felt nauseous and extremely tired after receiving the vaccine, which continued for two days after receiving tut. Three days later, I woke up struggling to breathe (but not anaphylactic), covered in hives, itchy all over, shaking, disoriented, and chilled but not truly feverish. I reported this to my allergist, who prescribed prednisone and suggested the vaccine combined with immunotherapy were too much for my immune system for one week. The hives, itchiness, and difficulty breathing lasted about four days. I struggled with fatigue and nausea for over two weeks, stopping all school and work in the meantime. After my second dose, I feel just a bit tired and sore, so I think this truly was a bad reaction to the two injections combined rather than to the vaccine itself. Still, it was a debilitating experience, and I would recommend considering putting out guidance that other immunotherapy patients space out their allergy injections and vaccines farther than I did.

Vaccination site all of a sudden got extremely itchy and sore, quickly became very swollen and red. Has gotten worse overnight and patient now has a fever

Fever ranging from 100.4-101.9. Severe body aches & burning skin. Shooting pains down legs and arms. Strange and painful pins & needles sensation in right food. Aches in bones and joints. Headache. Alternating freezing and heat flashes.

Premedicated the day before with Solumedrol, prednisone, albuterol, and benadryl. At 2:45 I started getting a tight chest, tight throat, itchy mouth, itchy throat, hives, throat clearing, and throat hoarseness/ fading. I then took benadryl and a breathing treatment every 4 hours. It got worse later in the day and even worse the next day. The next day I had body aches, chills, headache, brain fog, dizziness, fever, itchy mouth, itchy throat , hives, tight / thick throat, very tight chest, and difficulty breathing.

1/13 vaccination 1/14 At about 2am, I was working and I got this feeling i had a fever. I told my boss I didn't feel well. He noticed I had a rash on my chest up to my neck. Told to go to the ER. 103.2 temp. I felt feverish, almost like I had the flu. Administered antihistamine and Tylenol. Symptoms lasted for about 24 hours.

SYMPTOMS OCCURED DAY AFTER INJECTION (4/1/2021). PATIENT PRESENT TO THE PHARMACY 4/2/2021 922AM- LEFT ARM AT AREA OF INJECTION- RED, SWOLLEN, AREA WARM TO TOUCH, NO ITCHNINESS, AND SORE ARM. PATIENT ADVISED TO TAKE 2 BENADRYL TABLETS ONCE SHE GETS OFF WORK AT 2PM. PHARMACIST WILL REACH OUT TO PATIENT THIS EVENING TO FOLLOW UP.

3 cm round area on upper L arm. There is a nodular sense to the lesion. Painful to touch. No induration or surrounding erythema. Does not represent an allergy, rather a response to vaccine. Treated with Loratadine 10 mg tab, ice to area, and follow if not improving or worsening.

Felt normal until the next day at time listed. Currently experiencing migraine, sinus pressure, fatigue, chills,, pain and swelling at injection site, and tremors

-Sore in area of vaccination (since 7pm day of vaccination) -Weakness/Slight aching from the waist down to feet (since 11pm)

12 hours after injection there was increase swelling around the injection site and spreading into the Lt shoulder and upper back. Nausea, tightness in left neck and left upper back with swelling. Went to bed with ibuprofen and ice on swelling. Woke up 18 hours (5am) feeling sweating, difficult breathing, chest was tight with more swelling in left arm and left upper back. Got up to ask for help and fainted. Called father, mother we checked blood pressure was low (98/40), and was heart rate was low (46). Got her sitting up and blow pressure and heart rate returned to normal.

1st dose on 3/4/21 complained of left arm redness at the injection site raised area on 3/8/21, lasted 4 days.

1st dose Moderna adm 3/4/2021 developed mild flu like sx lasting 2-3 days. 2nd dose Moderna adm 4/1/2021 developed moderate flu symptom.. Left arm injection site swelled to size of a peach. On 4/3/2021 noted heart racing and chest tightness. Her pulse 90-120 by home pulse ox monitoring. Taken to ER by her mom. She had blood work, EKG and rec to f/u with her primary care physician for "adverse reaction to Moderna". 4/5/2021 had office appt. The PA stated an adverse reaction to the shot. Her pulse 133 in office. BP 138/70. She has received a referral to a cardiologjst. Continues to feel like her heart races, approx. 6 times today, with sharp chest pain or tightness.

Around the time of receiving the first dose, I began experiencing hives in a linear pattern. I have been getting hives everyday since I got the shot in my thighs, chest, abdomen, arms, and back. Begins by feeling the urge to itch and once I start itching, raised bumps in a linear pattern will appear every where. Symptoms are only alleviated by taking Zyrtec daily. If I go more than one day without taking a Zyrtec, I begin to break out in hives at random time throughout the day every day. I have no previous allergies to food, no changes in my lifestyle or diet either.

The patient's medical history included vasovagal condition that cause heart rate and blood pressure to drop due to stress on body and dehydration resulting in fainting. Concomitant product use was not provided by the reporter. On 09 Feb 2021, prior to the onset of the events, the patient received their first of two planned doses of mRNA-1273 (Lot number: 013M20A) intramuscularly in the left arm for prophylaxis of COVID-19 infection. On 09 Feb 2021, the patient was fine until about 11 pm after which she experienced arm soreness on the injection site and felt nauseous. On 10 Feb 2021, at 5 am in the morning, the patient fainted. The patient assumed that shot might have added stress to her body which most likely caused her to faint. No treatment information was provided. Action taken with mRNA-1273 in response to the events was unknown. There was no change planned to the dosing schedule of mRNA-1273 in response to the event as patient would be receiving second dose on 09 Mar 2021. The outcome of events, fainted (syncope), injection site soreness in arm (vaccination site pain), and nauseous (nausea), were considered as unknown.; Reporter's Comments: Based on the current available information and temporal association between the use of the product and the onset date of the reported events of syncope, vaccination site pain, and nausea, a causal relationship cannot be excluded. Patient's prior medical history of vasovagal reactions is considered a contributing factor to syncope.

21 year old female patient ambulated to Hospital with complaints of abrupt urticaria 2 hours after receiving first Covid-19 Moderna vaccine. Patient vital signs within normal limits. Physical examination was conducted with no signs of anaphylaxis. Patient was prescribed diphenhydramine 25mg capsules, taken 2 tabs by mouth under direct observation. She was monitored for an additional 40 minutes, with vital signs retaken and within normal limits. She was placed Sick In Quarters x 24 hours, and instructed to follow up with her DR should symptoms not improve.

Initially thought ringworm, for which I applied topical ointment prescribed by a Physician Assistant for but it did not get better and then my primary doctor informed me it was likely pityriasis rosea and that it would go away. It lingered until the second dose which was on 1/23/2021 but did not resolve and only got worse after the second dose, the rash finally resolved 3 weeks following the second dose.

Symptoms: Almost exactly after 12 hours, I started experiencing chills, fever, joint aches, and upset stomach. Symptoms went away over night, but arm at site of injection was very sore and painful.

Normal injection, no immediate side effects except for a sore arm that went away after two days. However, 7 days post-vaccine the injection site became itchy and painful to touch. The pain went away within a day, but the site remains itchy (10 days post-vaccine). The injection site now also has a light yellow bruise on it.

On 03-MAR-2021, prior to the onset of the events, the patient received their first of two planned doses of mRNA-1273 (Batch number: unknown) intramuscularly in the left arm for prophylaxis of COVID-19 infection. On 03-MAR-2021around 15:00, she received the Moderna COVID-19 vaccine in her left deltoid muscle at Shop Rite Pharmacy. 6 hours later she noticed red scratch marks all over her upper body, her heart beat was getting very rapid, her breathing was labored, her vision was wonky, her right eye was swollen and her face was beat red. 911 was called, upon arrival the EMT administered IV diphenhydramine (Benadryl) 50 mg and at the emergency room they gave her famotidine (Pepcid) and a steroid drip (methylprednisolone). After receiving treatment, the reporter states the patient felt great and was discharged. The doctor prescribed her methylprednisolone. Additionally she states the patient was also nauseous last night which went away around 2:00am. They told her to consult her primary care provider about the second Moderna COVID-19 vaccination Action taken with mRNA-1273 in response to the event(s) was not provided/unknown. The outcome of the event, serious allergic reaction is unknown.; Reporter's Comments: This case concerns a 21 Y/O F with serious unexpected events of hypersensitivity, dyspnea, and nonserious unexpected heart rate increased, vision blurred, eye swelling, erythema, scratches and expected nausea. Event onset on Day 1 after first dose mRNA-1273. Treated with diphenhydramine and methylprednisolone. Hypersensitivity resolved. Based on current available information and temporal association between use of the product and start date of the event, a causal relationship cannot be excluded.

Throat swelling, itchy tounge and lips, difficulty breathing, dizziness, full body hives, vision difficulty, hospital visit

Nurse made errors in administering vaccine. Lower-than-authorized dose volume administered. Fluid leaked out after removal of the needle. The fluid sat on top of vaccine site. Nurse also had trouble drawing vaccine fluid out of vial into needle, so I am unsure if the proper dosage was in the needle to begin with. Nurse pinched vaccination site, leading to potential subcutaneous injection. I did not see the angle or depth of which the needle entered. 3 hours after vaccination, large bump began to form on vaccination site. Has grown large as of time of writing, currently 1.5inch in diameter. Bump is hot and painful to the touch, and tender. Only other side effect is fatigue.

Large erythematous, warm, itchy, macular rash around injection site. Noticed 1 week after injection but believes it was present prior to this.

Papular dermatitis with erythema and puritis only on extremities, all 4. No rash noted on buttocks, abdomen, chest, back, neck or face. Worsening sxs over the last few days.

Seizures; Redness; Soreness; This spontaneous case was reported by a consumer and describes the occurrence of SEIZURE (Seizures) in a 21-year-old female patient who received mRNA-1273 (Moderna COVID-19 Vaccine) for COVID-19 vaccination. The occurrence of additional non-serious events is detailed below. The patient's past medical history included No adverse event (No medical history was provided.). On an unknown date, the patient received first dose of mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) 1 dosage form. On 29-Mar-2021, the patient experienced SEIZURE (Seizures) (seriousness criterion medically significant), ERYTHEMA (Redness) and PAIN (Soreness). At the time of the report, SEIZURE (Seizures), ERYTHEMA (Redness) and PAIN (Soreness) outcome was unknown. The action taken with mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) was unknown. No concomitant medications provided. No treatment provided. Reporter did not allow further contact

Pain and redness around injection site. Injection site is warm to the touch and pain extends to underarm.

I concluded my menstrual cycle a few day before the vaccine and two weeks after the vaccine I began my menstrual cycle again. I?m not sure it?s related to the vaccine but it has never happened to me before. So I thought it would be best to report since if heard similar things from other women. It?s heavier than usual and more painful. Also I had pain in my left armpit that went away after a few days that started about a week before my menstrual cycle. I?m guessing it was swollen lymph nodes due to the vaccine because I have seen other women report that as well.

Rash on finger, toes that spread to hands, feet. Immediately very itchy then swelling worsened and heels were painful when standing. Came to clinic on day 3; diagnosed with erythema multiform. Treated with prednisone x 4 days; follow up next day to review lab which only showed mild ESR elevation and improvement of symptoms.

Dermatitis on the right lateral side of the deltoid the following day after the vaccine was given. Rash continued to expand, swollen shoulder, was red, and sore. Rash eventually went away after 5-7 days without any topical creams/ medication. Arm soreness went away after 2 days.

About three hours after getting the vaccine, my left thigh started to get a bumpy, itchy rash. Within thirty minutes, I had a rash on both my legs and feet. I took 50 mg of Benadryl and it went away. I am still a little itchy, but it is much better.

Had all the regular symptoms the day after the second dose such as headache, nausea, bodyache, fever, etc. Two days after vaccine I woke up with hives on both forearms and a little puffiness under my right eye. Three days after I woke up and my right eye and cheek were super puffy and I had a rash on my face that was red and blotchy and itchy. It is now four days later and hives and face rash still persist but due to medications don't seem to be getting worse.

While waiting 15 minutes following vaccination the patient experienced tingling in her left arm that descended to her fingers and travelled to her face. She stated it felt like she bumped her elbow. Patient stated she had no trouble breathing. She later developed hives (within a couple hours), but they spontaneously resolved.

I got the Moderna vaccine on 04/13/2021 around 5:30 pm, waited 15 minutes in observation, by the time I got home around 7pm I had a really bad rash around different parts of my body

Caller states she received her second dose of Modenra on 4/14/2021 and on 4/15/2021 the area became red, sore and swollen. Caller does not believe it is warm to touch at this time. Caller states it seems to be gettig "redder", advised caller to have PCP or urgent care asses the site to r/o infection

Patient arrived to get her second Moderna Covid vaccine. Consent obtained and 0.5 ml of Moderna given via IM injection into LD. Pt went into adjoining room to wait her 15 minutes. She came out of room about 4 minutes later and said, "I don't feel right, something's wrong, I don't feel right." Her skin was very pink and flushed in face and I had her sit down. Nurse asked if it was difficult to breathe and she said "yes." Nurse asked if it felt like her throat was tightening and she said, "yes." Dr. was next in line to get her vaccination. Dr. palpated a pulse of 130 bpm while author called for assistance in overhead page of Medical Emergency. Nurse then readied the epi-pen for injection. Pt then was more pale in color and still talking, breathing without audible wheezes and maintaining an airway. Epi-pen not administered at this time as no loss of consciousness, and employee was still breathing/talking. Assisted by nurse and she obtained vital signs while updates given to another nurse and ED staff. Gurney obtained. Pt was able to pivot and stand to sit on gurney and she was taken to ED for further observation.

Painful lymph nodes under left arm and behind left knee; swollen, hard, and itchy area about 4in diameter near injection site.

Large swollen itchy red rash at injection site along with pain and fever/nausea plus feeling weak and it?s been over a week of putting Benadryl cream on it and taking that and Tylenol

Headache, chills, fever, arm soreness, skin sensitive to touch. Feels like i have covid again waking up this morning.

Around 12:00 am started getting a headache, a while after that started getting chills, then feverish, around 11:00 am woke up and still had all of those side effects. Took 3 Advil and drank a cup of water then slept until 5:30 pm. Once I woke up I still felt sick but I took 5 ibuprofen, ate a bowl of noodles and showered and felt perfectly fine. Around 8:30 pm I noticed a light rash forming around the injection site. Does not hurt or itch as of 7:32 pm April 18th

I noticed after removing the band aid that the spot where the shot went into my arm is now raised/inflamed a bit. It feels raised when I touch it.

Pt received covid vaccine. Medic reports as soon as she was stuck she started shaking. Pt with husband reports she has anxiety and is on meds. Pt able to talk continues to shake face red. Pt reports she takes Zoloft daily . After 15 minute wait pt was talking and laughing Face no longer red of shaking Pt reports she feels fine BP 126/78 HR 78 pt states she will report to MD

There was swelling for a few days that went down but suddenly today in the injection area it got worse and is like a golf ball sized lump that?s rock hard and itches really bad

Rash and swelling near injection shot about a week after receiving the shot. No rash or swelling initially.

My left arm developed a rash, my left eye is super itchy, and the rest of my body itches. Have not treated it. It began a week after I got vaccinated.

A raised red rash was noted the night of the vaccination. It is hard to the touch. Patient reports it does not hurt or itch. It is still there on 4/23.

Redness in the face an neck, slight swelling. Mouth going numb. Both lasting only 30 seconds to a minute. Happened 20 minutes after the shot.

Within the first 24 hours a hard uneven lump (about 1.5 inches in diameter), heat around/on injection site, and soreness, and mild itching developed. By 48 hours, soreness peaked and the lump remained with warmth and itching when the area was touched with slight pressure. Between the 2nd and 14th day, the lump was still present (still 1.5 inches in diameter) with mild itching. I began to develop a dry cough, shortness of breath, severe fatigue, and muscle fatigue that lasted from day 5 to day 11 following the Moderna injection. I went to get a COVID test and the results came back negative. After the 14th day the lump went down but now there is a discoloration of the skin where the lump was located that is still present 23 days after the first injection. I did not use any medications or treatments to heal the adverse event nor sickness symptoms following the injection.

Pityriasis rosea breakout. april 15 had fever of 102, it went away and by the 21st i have developed a rash that looks exactly like Pityriasis rosea. started with one big spot and now is breaking into many.

7 days after patient had her first shot she started developing a rash that has involved 85% of her body.

pt received 1st dose of moderna vaccine on 4/18/21 at store. She reports no sxs until the morning of 4/26/21. Pt woke up with red, itchy spot at injection site, reports this is her only symptom. She said she will take cetirizine to help with itching. We contacted her the next day on 4/27/2021 and she reports the spot is getting smaller and less itchy, we will continue to check in with her until this resolves.

At 10:40, the patient went in for her first Moderna vax. She was slightly nervous since she's not a fan of needles. When she got it a vessel must have been nicked, because she experienced a bit of blood streaming down her arm which surprised us. The nurse wasn't concerned and stopped it quickly and cleaned up her arm. Got her a new bandaid and she was sent to the chairs in the waiting area to wait her 15 minutes following the vax. I had to pick up my regular prescription so I got in line while she was sitting there surfing on her phone. As I got to the front of the line, the patient called me and said she was feeling lightheaded. I immediately left the line, and was there (I was maybe 15 feet away) and was asking how she felt. She said she felt sweaty and like she might pass out. Right then, her eyes rolled a bit and closed and she went limp. I was able to hold on to her and a customer that was behind me in line and I were able to alert the nurse that gave the vax and the pharmacist. Both immediately came to our aid. We got the patient on the floor and on her side. The nurse wanted us to have an epi pen in case, but we all didn't think it was an allergic reaction. We checked her blood pressure and it was low. Another employee was already calling an ambulance and another was getting her some juice. She was beginning to regain consciousness. She was only out for about 2 to 3 minutes at the most. Her blood pressure was low, but started to come back to normal pretty quickly. We had her lay there a few minutes and then sit up. Eventually we moved her to the chair a few minutes later. Within another few minutes the emts arrived. She had already had answered questions for us like her name, birthday, where she was, etc. The emts repeated those same questions, asked us what happened and rechecked her vitals. All were back to normal. They gave her the option of be transported to the ER, but given that she was feeling back to normal she declined and everybody felt good about that decision.

?COVID arm? - rash appeared on site of injection approximately 7 days after the first dose of Moderna. It is not itchy or painful, just a large slightly elevated red mark on my upper arm. It is still currently present (7 hours later). No other symptoms or side effects.

Patient presented nine days after vaccination with redness, swelling, pain, warmth, induration, and a spreading rash around the injection site. Patient reported that she first noticed the symptoms about 24 hours prior to reporting to clinic for evaluation. Patient presentation was consistent with injection site cellulitis with probable cause of vaccine being given via the subcutaneous route.

Initial arm pain that went away after 48 hours that spread up the neck and a headache in the first 24 hours. However, it went away and then yesterday (April 28) I noticed the arm soreness returned. This morning (April 29) the spot was itchy and more sore. This evening it is somewhat swollen and very red, continues to be itchy and aches.

Hives on injection site, itchy and red. About 2 inches wide and 3 inches tall. Started April 30 around 12:00am and is persisting until May 1 around 2:30pm

Per EMS, patient with no history of vaccine reactions felt "dizzy" and had hives on chest post Moderna vaccine #2. BP 142/87 1422 and did not normalize during course of care. No reported reaction to first vaccine. 25 mg diphenydramine IM, transport at patient's request. Patient counseled to consult provider before receiving any additional booster vaccines.

I received the vaccine April 30th. I had intense pain in arm until it subsided a bit two days later (Morning of May 2nd) and there is a large red circle around the injection site (bigger than a Quarter coin) that itches. The circle began to appear May 1st, 1 day after injection.

Initially with angioedema lips and eyes. Then hives developed. Went to Urgent Care and was given oral methylprednisolone and antihistamines. Seen in our office today for follow up. Stable. No shortness of breath or wheeze. No chest pain. No abdominal symptoms. Never had allergy in the past. No lip fillers.

Noticed swelling/red "spot" around vaccine site on the night of 5/6; did not hurt or itch but was warm to the touch and about 2-3 inches diameter. By night of 5/7, "spot" was still swollen, slightly itchy (if shirt rubbed it too much), and still did not hurt but had spread to about 4 inches in diameter -- more ovular (longer horizontally than vertically).

Exactly one week after the shot, I got a bit of covid arm meaning the injection site was visibly red but there was no swelling or itching. Now, starting yesterday, I am dealing with all-over body hives. Yesterday much of my neck was swollen and bright red. Today my hands are swollen and bright red with hives elsewhere on my body. I?ve been taking Benadryl.

At approximately 6:00PM Tuesday May 4 (approx 8 hours after second dose): Fever up to 101.6 F, severe chills, muscle pain at injection site, profuse vomiting ( at least 7 times between 10:00PM and 4:00AM 05/05), muscle spasms in upper legs. Severe stutter On Wednesday May 5, starting around 3:00PM: Persistent stutter, full body jitters that progressed to upper body muscle spasms (primarily in the neck and shoulder region) that worsened throughout the evening. Checked into the ER approximately 4:30PM and was administered a muscle relaxant (I believe) through an IV to lessen the spasms around 7:00PM. Muscle spasms and stutter persisted until approximately 4:00PM on Thursday, May 6 at time of discharge from hospital.

Patient reported hives, swollen eyes and vomiting about 1 hour after Moderna dose was given. States that symptoms lasted over 24 hours before resolving.

Patient states her arm was swollen, red, hot to the touch and itchy. She stated she had fever along with rash.

Hot face and neck, nausea, body aches, fatigue, extremely sore arm around injection site couldn?t lift it

I got my shot Thursday afternoon... beginning while I was sleeping that night until I woke up I was freezing and then woke up sweating. On Friday, I had a headache when I woke up as well as body aches and pains. Additionally a large swollen, red, warm spot appeared at the injection shot. It is very itchy. I googled it and found that it is considered ?Covid Arm?. Saturday I still had a headache and achey body and both friday and saturday i was very tired. I also developed a deep, junky cough on Friday which I think was due to the shot.

Pt received the vaccine on the 7th of May. 1 week later she noticed a red, mildly raised rash at the injection site. Pt denied any pain itching or burning at the site. Pt denies any prior similar reactions to vaccines. She has not had the rash for 4 days with mild improvement. No other associated symptoms to include difficulty of breathing, SOB or diffuse rash other than injection site were noted by the Pt.

Woke up the next morning with numbness in injection arm from shoulder to elbow. Lasted about an hour after waking up. (Till about noon) Painful to move arm about two days after injection. (Note, I did have COVID-19 in January)

Patient presented to pharmacy on 5/19/21 showing large rash around jawline and another similar rash on the back of the leg. Looks similar in pattern to ringworm- large round welts. Did not seem to be raised but was pink in color. Patient said they were itchy. This began 1 day post vaccine on 5/3/21. She went to an urgent care on 5/6/21 when rash was not going away. She was prescribed clotrimazole/betamethasone cream and cetirizine. As of 5/19/21, rash still persists. She had concerns regarding what would happen when she got her second dose.

Soreness in injection site beginning around 3 PM on the 18th, with fever-like symptoms (hot and cold, soreness and sluggishness) starting around 3 PM and becoming worse throughout the night. Woke up on the 19th at 7:30 AM with continued injection site pain and fever like symptoms but no notable fever. Also began to feel nauseous on the morning of the 19th, which continued all day. Woke up on the 27th at 10:00 AM feeling mostly back to normal with a small amount of injection site pain left over.

Patient reported itching at the injection site within a few minutes of the injection. At that time she had no other symptoms. A few minutes later she reported generalized itching and nausea. She did not have a visible rash. She continued to deny any shortness of breath, trouble swallowing, or swelling of the tongue or throat. She stated that her current symptoms (itching and nausea) were similar to those she had experienced with past serious allergic reactions and agreed to receive epi. Epi was administered via epi pen at 12:36 PM, EMS was called at 12:37 PM. A few minutes after epi was given, she still did not have a visible rash but began gagging and appeared to struggle on inspiration. Although patient had a glazed expression she was still able to focus her eyes and respond yes when asked if she could breath. She did not vomit but still reported feeling nauseous. EMS arrived at 12:48 and transported her to the ED. Per review of ED records she received an additional dose of epi in route to the hospital. Per hospital report, she received diphenhydramine, methylprednisolone IM, and ondansetron. She was monitored for 4 hours and discharged in stable condition.

PATIENT CALLED ON MONDAY 05/17/21 AND STATED THEY HAD A RASH ON TRUNK AND BACK AND A FEVER OF 101.8 ADVISED TO TAKE SOME TYLENOL AND BENADRYL AND NOTIFY THEIR PHYSICIAN

40 mins (3:10 pm) after injection, Hands started feeling like they were on fire, discolored, veins over thumb areas were enlarged and swollen. Swelling went up to forearms. Face was flushed and started to swell. Feet initially swelled and then became numb and cold. Went to ER as I'm anaphylaxis to Benadryl. Was given 125mg of IV steroid and 40 mg of IV pepcide @ 3:45pm. Allergic rxn symptoms subsided within 20 minutes of medication. Was given a 3 day dose take 60 mg of Prednisone.
