Moderna

severe left sided testicular pain, lasting approx 3 hours. Pt unable to stand or walk for several minutes after waking up. After using the restroom later, pt felt some pain diminish, but is still tender. Scheduled visit with Dr to evaluate on 6/22/2021. Suspected Testicular Appendage Torsion. Today (6/23/21) small blue "bruise type" spot on upper left testicle.

The patient received one dose of the Covid-19 Moderna vaccine at a mass vaccination event on 2/5/21. The error was discovered on 3/1/2021 when the patients mother was trying to schedule her sons second dose. A telemedicine visit was scheduled with mother and Moderna was consulted. We explained to mother what had happened, and based on the recommendations from Moderna he should not proceed to the second dose. Other than arm soreness at the injection site that lasted one day the patient did not have any other adverse effects or complications.

Unauthorized age group. Received first dose of Moderna vaccine. Received vaccine on 07/27/2021. Follow up phone call to parent on 07/28/2021. Patient had only c/o soreness at injection site and reported he had slept on the side the injection was given on.

Brief syncopal episode, fell from chair, struck head on floor, suffered small laceration and abrasion to left eyebrow.
