This form is to report the second dose of the vaccine shot was mistakenly administered to a person (info above) that was under the age of 18 y/o by me on 02/02/2021. There were no adverse reactions, allergic reactions, or any symptoms other than mild soreness at the injection site, confirmed with the individual receiving the vaccine. I was advised to administer the 2nd dose to the same person by Vaccine POD supervisors. This action of administering 2nd dose to person under age 18 y/o was approved. Pt had no symptoms during 15 minute observation.

My son is 16 years of age and was given the Moderna vaccine despite it only being approved for patients over the age of 18. He is experiencing mild fatigue post injection and some soreness at the site. My main concern is the second injection that is scheduled for April 15th at the same location. Should he continue with the course and get the 2nd shot of Moderna? I have been trying to speak to medical center regarding this because not only was his age stated at the appointment and he was found in the medical center system, but he provided id to a woman with the title of director who did not catch it.

16 year old male , volunteered with his mother at COVID vaccination event - received vaccination at event. His parents approved of vaccination - he did not disclose age prior to vaccination . Would like to proceed with 2nd vaccination - parents approve . Mother spoke with pediatrician who would like patient to receive 2nd dose. Minor consent to be signed Minimal arm soreness post injection

Client originally given the 1st dose of Moderna on 05/07/2021, by previous healthcare worker, but client not in the age range for the Moderna vaccine. Per Dr. client was to receive the 2nd dose of Moderna, per off label use. Client denied any adverse reactions, denied difficulty breathing, denied itching, and reported just pain at injection site. Mom also present and denied all adverse reactions.

In January, staff were administered the COVID vaccine. After administering to this patient, the immunized realized they calculated their age incorrectly. Patient was two month under the age of 18, his mother was available to give consent. Patient experienced headache and soreness at injection site.

Vaccine age contraindication; 17 year old patient was injected Moderna brand COVID 19 vaccine which is approved for patients 18 years and older. Vaccination occurred on 03/10/21. Patient's mother was contacted on 03/11/21 for follow up. Patient's mother stated that the only side effect patient has experienced is pain on the injection site.

Left arm has rash at injection site. Hard itchy ball bump. Similar to a mosquito bite that has swollen up.

Patient was experiencing soreness at the injection arm for about 2 days with no other complications.

PATIENT RECEIVED HER 1ST VACCINE ON 3/23 REPORTING SLIGHT FATIGUE AND SORE ARM AT THE INJECTION SITE. SECOND SHOT WAS COMPLETED AND PATIENT WAS ADVISED TO FOLLOW UP WITH PEDIATRICIAN. PATIENT WAS ACCOMPANIED BY MOTHER FROM THE FACILITY.

MEDICATION ADMINISTERED AS AN "OFF-LABEL" USE DUE TO PATIENT AGE. PATIENT ONLY EXPERIENCED MUSCLE SORENESS IN INJECTION ARM.

Patient received vaccine out of the recommended age rage at age 17. No adverse events reported. Reports mild pain to injection site for 1-2 days, no other side effects.

Given to individual less than 18 years of age. Father states child had sore arm from injection but no other complaints.

The person is an emt and arrived to get a vaccine. The nurse triaged and administered the vaccine. Vaccine administration to a person below the age for EUA approval was discovered during a routine chart audit and looking for persons who did not complete immunizations for COVID. this was discovered on 5/14/2021. I spoke with the mother ( since person is a minor) and informed her of the vaccine error. The person reported that the person who received the vaccine did only experience local side effects for 1 day of pain at the vaccine administration site. the mother reported that the patient returned to his regular duties.

Patient, aged 17, was inappropriately given Moderna COVID19 vaccine, not recommended under the age of 18. Pharmacy attempted to contact patient immediately upon realization, roughly 3 hours after vaccination. Pharmacy finally reached patient's mother the next morning, and she was informed of the error. Patient's mother will monitor patient, and in accordance with CDC recommendation, the pharmacy will plan to administer the second dose of Moderna to the patient at the appropriate time. Patient's mother agrees with this plan. At the time of contact, patient had only developed mild soreness at the injection site.

Red, blotchy rash forming on and below chest and neck appearing about an hour after vaccination. Not itchy. Resolved within an hour.

- 3 hours after vaccination: Tensing occurred around the injection site. Pain can be described as fatigue, making it painful to move the muscles around the area of injection. The symptom lasted for two days. - 0215 the next day (1/26/2021): Headache occurred and lasted for approximately 36 hours.

Pain lasted for 3 days. - 1430 of 2/25/2021: A headache occurred that lasted for approximately 24 hours. The headache got worse at around 1600.

31 y/o male came in for first dose of COVID-19 Vaccine-Moderna Lot# 013A21A exp:04/04/2021. After patient completed his 15 minutes in the observation he verbalized to check out OA he wished to speak to a Nurse. Upon arrival to patient, he was complaining of itchiness to his neck and back. Redness to his forehead and neck noted. Initial vitals O2sat-100%, P-102, Bp-132/86. 1533- One 50mg capsule given with water. 1539-V/S130/86 99% P-98. No SOB noted nor reported by patient, A/O, No compliant of breathing by patient. Redness noted to be less to forehead and neck. 1543-bp-126/80, p-70 O2-100%. Patient verbalized feeling better, slight redness noted to neck only. 1550-118/70 P-76, O2 99%. Patient called his mom who gave him a ride home.

patient "saw black"and nearly passed out was grey, sweaty HR 39, BP 96/60 after a few minutes his color came back after vomiting x2 observed in tent, BP 136/78, HR 69

developed a rash on his right elbow about 9 days after vaccine. It then appeared on his hands and left elbow. It has since started to clear up.

This morning I woke up and had a knot at the injection site, along with itchiness. We are 8 days out. I read online that it is considered delayed cutaneous hypersensitivity. I have had zero problems regarding the Moderna vaccine, and this isn?t an issue at all. It is just a small symptom. I figured you guys would be interested in hearing about it. I read that it was more prominent in women and was not that common. I am perfectly fine, like I said.

immediately after receiving his vaccine, he swelled at the injections site. His arm was red. After 10 minutes, he had a red rash around the injection site. After 10 more minutes, injection site was red and rashy, after 10 more minutes, his injection site reaction was mellowing out-no additional redness or swelling. He felt fine each time I asked him-no itching or trouble breathing. After 30 minutes, he left with his mother. I called him at 2:45 pm and he was feeling fine. he said that the rash and swelling were going down but his arm did hurt.

Aside from usual soreness of the site, near constant muscle twitches in nose and/or right thigh that were never present before. Just a minor thing that I figured was worth reporting due to it not being a listed side effect.

15 minutes after Moderna injection, patient began to experience "throat tightness" and headache. Slight rash noted on neck area. Nursing staff responded and administered EpiPen 0.3mg IM right vastus lateralis and Benadryl 50mg IM Left deltoid once. Ambulance was called to scene and patients symptoms had begun to resolve. Patient remained alert and oriented and did not require additional support. Blood pressure was 122/90 prior to Epinephrine. Per ambulance staff, patient was transported via parent to the ER for further observation.

Headache and mild body ache/fatigue. Soreness and discomfort at injection site. Almost all symptoms gone within one day.

Patient with generalized urticarial rash and eye swelling, no respiratory distress, no other systems involved besides skin, no mucous membrane involvement, taken to ER, treated with prednisone and benadryl, improved, sent home with benadryl, rash resolved in 48 hours

Patient received the vaccine and was instructed to sit for 15 minutes as required before leaving. About five to ten minutes to, we were alerted that the patient had fainted. Upon coming to the patient, he was starting to come to but was out of it. I tried asking the patient how he was feeling, is he having a hard time breathing? Patient's entire arms and legs were bright red with possible rashes that lasted about 5 minutes and went away quick. Patient's face remained their normal shade. Patient reported he fell asleep and patient's father reported waking the patient up prior to coming to get his vaccine. Patient sat at the pharmacy to be monitored for an addition 30 minutes before leaving with father. Patient was given water and fan to help cool him down. Patient reported feeling ok after about 10 minutes with legs elevated but was instructed to sit for longer than that time. Patient was ok to leave with father after 30+ minutes of monitoring.

At the injection site a big blister ended up forming and is quite painful to the touch as well as the expected muscle pain, as well as the next day it felt like I had gotten hit by a truck, about every muscle was sore in my body and I was running a low fever

Red, swollen, warm to touch, and itchy at site of injection. About 1.5" X 3" area. One tablet of Benadryl taken. Red and itchiness went away. Still swollen and warm to touch. Also, eyes were dry and nose was runny.

It was described as a full body rash that resolved without permanent effect. It was recommended to not get next dose based on symptoms. A licensed physician present during the clinic agreed not to receive the next dose at this time.

Patient received 1st dose of vaccine, about 5 minutes later patient fainted/passed out in the chair during the observation period. Hit head on floor and had a hematoma form. 911 was called and patent went to hospital for observation. Blood pressure was running low after fainting in the 90/60 range. When patient came too said this happens all the time, was asked about issues with previous vaccines before shot and said no issue and marked on cdc checklist and consent form no issues to vaccine.

Small red bumps appeared the evening two days after reviving the vaccine. They do not itch and they have seemed to calm down 24 hours after they appeared. They are semi swollen and they are scattered across my arms

The occurrence of additional non-serious events is detailed below. The patient's past medical history included No adverse event (No relevant medical history reported.). On 02-Apr-2021, the patient received first dose of mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) 1 dosage form. On 03-Apr-2021, the patient experienced VACCINATION SITE PAIN (sore arm at injection site). On 04-Apr-2021, the patient experienced FATIGUE (some fatigue). On 07-Apr-2021, the patient experienced DEPRESSED LEVEL OF CONSCIOUSNESS (seemed to almost fall asleep and become unconscious like) (seriousness criterion medically significant) and HYPOGLYCAEMIA (hypoglycemia). At the time of the report, DEPRESSED LEVEL OF CONSCIOUSNESS (seemed to almost fall asleep and become unconscious like) and HYPOGLYCAEMIA (hypoglycemia) had resolved and VACCINATION SITE PAIN (sore arm at injection site) and FATIGUE (some fatigue) outcome was unknown. The action taken with mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) was unknown. Concomitant product use was not provided by the reporter. On 03-Apr-2021, patient took Tylenol for the sore arm. On 09-Apr-2021, patient went to his doctor. He got some blood work done. His A1c was fine, his fasting glucose level was fine. Doctor doesn't think it's diabetes but had an episode of hypoglycemia. He also did not rule out possible side effect of the vaccine. He has not had another episode since then. Caller is asking if there have been any other reports of hypoglycemic episodes and also asking if Moderna would recommend getting the second dose given the reaction he had with the first dose. This case was linked ( Linked Report).; Sender's Comments: Based on the current available information and temporal association between the use of the product and the start date of these events, a causal relationship cannot be excluded.

Patient received Moderna Covid-19 vaccine and fainted approximately 5 minutes later. Patient reported feeling dizzy, hot and nauseous. Patient's blood pressure 81/41mmHg, heart rate 58 bpm, oxygen level 96% (taken by paramedics) approximately 10 minutes post-vaccination. Paramedics took patient to emergency department to be monitored. Patient was given IV fluids and discharged from the hospital a few hours later. Patient reported that he has a needle phobia and had fainting incident post-vaccination years ago.

T+0:00, second COVID shot received. T+2:00, My arm is starting to inflame and firm up. It?s painful, but bearable. T+5:00, Mild headache, but again bearable T+6:00, Chills and fever set in, headache intensifies, it was around this time I took 400 miligrams of ibuprofen to combat the now throbbing headache. I spent eight hours in bed and I felt fine, but standing up and moving about made my headache come back. T+14:00, I took 400mg of Ibuprofen, drank some water, and decided to try to go to sleep. T+16:30, I woke up with chills, but I managed to go back to sleep. T+20:45, I woke up again, same symptoms, went back to sleep. T+25:15, I woke up and took another 400mg dose of Ibuprofen. T+36:00, This is when I noticed I stopped feeling chills T+48:00, headache symptoms are almost gone.

hives started 24 hr after inj, first on face and scalp, subsided by 72 hr. March 30, pt on vacation at the beach, "rash" on trunk, attributed to sunburn. Relieved with diphenhydramine and gone ~12 hr. April 4, "rash" reappeared trunk-back/chest, went to urgent care, referral to dermatology. April 7 seen by dermatology, cetirizine bid, triamcinolone ointment prn,benadryl at bedtime prn if needed, diagnosed as hives. Continued with "flares". Nothing specifically identified as the cause? Mom with hives as well(reported in separate form). Only common denominator, Moderna vaccine. No hives for pt's father(vaccinated with Pfizer) or sister(15 y/o, no vaccine yet). No changes to home environment. No improvement in s/s. Seen by dermatology for f/u April 17, increase to cetirizine q 6 hours w/ one month f/u and potential for prednisone taper if needed. Now, May 8, hives are much improved, still on cetirizine q 6 hours. No prednisone used yet. Follow up with dermatology on May 17. Of note, when dose of cetirizine is delayed... symptoms come back.

Patient experienced soreness at injection site, high fever (102 for 3 days), severe headache, muscle pain, nausea and vomiting, stomachache, extreme fatigue, palor. All these symptoms persisted for approximately 7 days. On the 8th day, patient developed strep throat as diagnosed by his physician and was given antibiotics.

On the Night of the day after I received the vaccine, which was about 24hours after, I started noticing a rash that covered all of my upperbody, being most prominent on my torso, chest, and back. The rash also spread to my upper legs, and looked like red splotches. I did not have hives are any large bumps, but the rash did have a little bit of texture, and it was slightly itchy at times.

Patient developed urticarial rash 1 week after receiving 1st dose of Moderna vaccine. Has had 4 ED visits for worsening urticarial rash despite receiving treatment. Saw patient in ED tonight with rash and throat tightness concerning for anaphylaxis, responded well to medications.

Patient reporting itching, hives, and rash on same arm as injection. Hives resolved with benadryl and time, however, rash is still present.

This is a rash on her left arm encircling this spot check the vaccine Covid 19 and she had the vaccine 1 week ago in the retroperitoneum a day or 2 of that and is now fading away

Rashes appeared on various part of the body starting Saturday morning. Gradually spread to other parts while subsiding in others. Rashes caused mild itch. Took a bath with bicarbonate soda and oatmeal. Rashes had mostly cleared by next morning and completely cleared by Monday.

COVID arm - Client stated that his arm became red swollen, hot to the touch, in the arm that received the injection. Client stated that it lasted about a week.

Pt was administered 1 dose of adult epipen and 911 was called. Once EMT arrived they took him to the hospital to be evaluated. He appeared to recover by demonstrating the ability to stand up walk around and respond to questions asked by the EMT.

Patient reported Tiredness, headache, and nausea on day of vaccine (7-25-21). On 7-29-21 pt woke up with body rash.

Patient initially had muscle soreness in the left arm for about 4 days which did subside. 2 days later he developed an enlarged axillary lymph node which continues to be painful and a 3" x 2.5 inch erythematous patch at the injection site. All symptoms are slowly dissipating but do continue at this time. No constitutional symptoms.

I got the injection at 3:35pm on 1/20 in my left arm. Within the first 30 minutes, my shoulder around the injection site was incredibly sore, my left elbow had a lot of joint pain, and my left thumb and index fingers went numb within the first 30 minutes, I woke up at 8:00am on 1/21 and noticed that I had a light headache, felt extremely groggy, and was experiencing an uncomfortable amount of muscular and joint pain. I went to work at 9:00 and on the drive there, I began to feel dizzy and I felt as if when I turned my head, my vision would lag behind and take a second to process my surroundings. By 10:00, my muscular and joint pains were significantly amplified, my headaches were increasing in frequency and amplitude. I also began to have the chills, followed by periods of radiating heat from my body. Coworkers told me I looked very flushed, however, I never developed a fever. By 11:00, the muscular and joint pains, coupled with the headaches forced me to sit down and I almost broke down into tears. There were three sperate times where I began to get very lightheaded and thought I was going to pass out ( I did not). I went home at noon and noticed that my muscles felt like they were on fire. I took Tylenol and Advil whenever I could, which helped to reduce the symptoms. we had dinner at 7:00, however, I had no appetite, without eating since noon. On 1/22, I woke up at 8:30 with a pounding headache, muscular and joint pains still there, however reduced, and the same groggy feeling. This morning, I also noticed that I had a very sharp stomach pain in my abdomen. I also woke up with a severe nosebleed, which is extremely uncommon for me.

On second day after vaccination, Patient began to notice cutaneous lesions on bilateral dorsa of feet and both palms. The dorsa of his feet had a slightly raised, pruritic erythematous rash that, over the course of the day, extended in places near to the ankle. Though not warm to the touch, Patient explained that they felt hot and painful. There was slight swelling. By 1/24/21, there was less swelling, and discomfort and the erythematous rash had become eczematous in places and thicker than the previous day. The palms didn't have an appreciable rash. However, Patient felt much discomfort there (not exactly painful) and some paresthesia. He felt that his fingers were swollen. By 1/24/21, the discomfort in both palms was worse.

Immediately after the vaccine was administered I experienced the usual side effects of mild headache, fatigue and localized redness and swelling Around 11pm the night the vaccine was administered I developed a migraine that resulted in blurred vision. and when I checked my temperature it was approximately 104.9 degrees Fahrenheit.By the next morning, approximately 7am, I was weak to the point I could not get out of bed myself, I was nauseous that resulted in a single Emesis episode approximately 8am, I had mild swelling of my face since 7am, between 7:30am and 12pm, I had multiple times where I would temporarily lose my vision. Throughout 1/25 I continued to have a fever ranging from 102 degrees Fahrenheit to 103 degrees Fahrenheit. At approximately 8am and 1pm I took two acetaminophen pills each time which alleviated the migraine and to an extent helped with the visual deficit episodes. Throughout 1/24 and 1/25 I suffered with extreme chills resulting in shivering that lasted for several minutes despite a room temperature of approximately 90 degrees Fahrenheit and covered in two blankets. PCP was consulted via telephone and she advised to go to the hospital via EMS given the high grade fever, abnormal vitals and severity of symptoms

I called my insurance provider's RN hotline at the onset of dizziness, which was then followed by a bout of ear ringing and nausea. I was advised to seek immediate medical attention, so I had an RA at my university dorm walk with me across the street to M Health Fairview hospital. I was seen by an RN in their emergency room, but was ultimately never seen by a Doctor as I did not present a clear risk of anaphylaxis at the time. After waiting in the ER for four hours for medical care, I ultimately left because most of my symptoms subsided. My most lasting symptoms cleared up late yesterday (1/25) and they were extreme soreness/tenderness at injection site and fatigue.

Received COVID-19 vaccine (Moderna) on 1/21/21. No local reaction. No immediate symptoms (hives, lip swelling, respiratory symptoms). One day later, feverish T99.5F. 1/23/21, Patient developed a painful red rash on the palmar aspect of his hands and dorsum aspect of his feet and has had this rash since (total of 5 days). He currently takes Xyzal daily. Father would like to know if patient should receive the second dose of the vaccine. Erythematous macular rash limited to hands and feet that is blanching. As time progressed, raised in some places. Looked eczematous in some places as well. Painful, stinging skin, and swollen. He has limited range of motion. Hot shower decreased discomfort, increased range of motion. Cold water also helped. Symptoms are improving. Spring 2020 - may have had COVID - had anosmia. Was not tested at the time, but did have COVID antibody testing in July which was negative. Family has reported the reaction to VAERS and is also using V-Safe. Patient has no history of reactions to vaccines in the past nor has he had an allergic reaction to any component of the Moderna/Pfizer vaccines. He also does not have PEG/polysorbate allergy history (though he has taken Miralax in the past for constipation). Patient does have a history of anaphylaxis.

Hives at injection site starting 7 days prior. Swollen lymph nodes side of injection. Red area and warm to the touch at injection site starting 7 days prior.

Onset of warm, red, rash at injection site initially, then gradual development of secondary rash below injection site, gradually enlarging, measuring 14 x 14 cm with serpiginous margins, clear center, erythema migrans appearing rash, no scale in center. Full ROM arm, no fever/chills, mild pruritus.

I received my first vaccine as the Pfizer manufacturer, and did not have any side effects. However, when I received my 2nd shot, I received the Modena manufacturer, and have experienced many concerning side effects. Fever, cold chills, trembling, fatigue, that lasted 2 days after the vaccine was administered. The side effect I am still experiencing is a metallic taste in my mouth, that gets worse when I drink water or other beverages. My hands and fingers turned a rust orange color the day after getting the shot and resolved within 48 hours.

Patient started not feeling well the night after receiving vaccine. Next day patient had flu like symptoms, headache, anxiety, increased heart rate and trouble breathing. Patient also presented with rash located back of neck and back - red, painful, burning and itching rash - painful to sleep on back the night after receiving the vaccine

Nose is stuffy; Trembling; Metallic taste in mouth; Hands and fingers turned a rust orange color; severe body ache; Headache; body chills; fever of 102F; Fatigue; Received first shot of Pfizer vaccine and scond shot of Moderna's; A regulatory authority report was received from a healthcare provider concerning a 19-year-old, male patient who received first shot of Pfizer vaccine and second shot of Moderna's/wrong product administered, headache, nose is stuffy/nasal congestion, chills, trembling/tremor, fever of 102F/pyrexia, hands and fingers turned a rust orange color/skin discoloration, metallic taste in mouth/dysgeusia, fatigue and severe body ache/myalgia. The patient's medical history included allergies to codeine and anxiety. Products known to have been used by the patient, within two weeks prior to the event, included Fluoxetine 40mg, Lamotrigine 100mg, Amlodipine 10mg and Lunesta 3mg. On 01 Feb 2021, approximately one day prior to the onset of the symptoms, the patient received a dose of the mRNA-1273 (Batch number: not provided) intramuscularly in the right arm for prophylaxis of COVID-19 infection. On 01 Feb 2021, the patient went to pharmacy and was offered his second dose of the vaccine, at the time of administration, the patient stated he was unaware that the vaccine given to him was from Moderna. His first vaccine was from Pfizer. On 02 Feb 2021, the patient stated he felt very sick, he had a headache, nose was stuffy, body chills, severe body aches, a fever of 102 degree Fahrenheit, and noted rust colored spots on his hands. The patient stated these symptoms lasted two days after the vaccine was administered. The patient also stated he had a metallic taste in his mouth that worsened when he drank water or other beverages. At the time of this report, the metallic taste in mouth had not resolved. The patient visited Urgent Care and was instructed to take a baby acetylsalicylic acid once a day. Action taken with mRNA-1273 in response to the events was not applicable. The outcome of the event, metallic taste in mouth/dysgeusia, was considered not resolved. The outcome of event, received first shot of Pfizer vaccine and second shot of Moderna's/wrong product administered, were considered resolved on 01 Feb 2021. The outcome of events, headache, nose is stuffy/nasal congestion, chills, trembling/tremor, fever of 102F/pyrexia, hands and fingers turned a rust orange color/skin discoloration, fatigue and severe body ache/myalgia, were considered recovered on 04 Feb 2021.; Reporter's Comments: Based on the current available information and temporal association between the use of the product and the start date of the events, a causal relationship cannot be excluded.

Patient started feeling itchy on face and arms. There was a rash present in both areas. This happened shortly after administration. Within a half hour of Bendaryl administration, symptoms cleared and condition improved.

one week later the patient had a large red circle that was extremely itchy and warm. It does not hurt.

While in observation area. Patient complain of itching and redness to bilateral arm. vitals 125/73 HR 76 02:96%RR 18 Brought to nurse station. complain of new onset of throat itching Benadryl 50 mg PO administered. Observed for additional 40 mins. patient reports "throat itching " resolved and improvement of bilateral arm itching . Bilateral arm remain with small areas of redness which has improved since initial assessment. d/c home no acute distress ER precuation Pt verbalized understanding (Patient verbalized is following an Allergy specialist that cleared him for initial vaccine administration, patient educated to follow Allergist prior to second dose administration)

During post vaccination wait time. Patient noticeably anxious/restless/bouncing legs. Initial assessment patient reported feeling like he had a hot flash and arms and were itchy. Vitals taken every five minutes, potential seizure activity noted, patient positioned for safety on cot. EMS activated, hives noted on left upper shoulder and left side of face. Patient complained of shortness of breath and crying. Patient became pale and LOC and temporary apnea noted, 0.3mg EpiPen administered in left leg x 1 dose, by RN. After administration patient gasped became combative. EMS arrived and patient transported via EMS to ED.

Developed "moderna arm" (raised inflammation with itching 1.5-2 inches long and 0.5-1 inches across) 7 days post vaccination from injection 1. Developed somewhat severe tension headache on day 1 post vaccination from injection 2 (all further events listed below are from injection 2). Headache responded to ibuprofen. On day 4 post vaccination, began experiencing minor and infrequent migraine-like headaches that lasted for a few seconds to a minute. Moderate pain and localized. Pain site changed but was generally on left side. Experienced 0-2 a day until day 12. On day 11 post vaccination, began experiencing full, moderate tension headache. In addition, had more frequent migraine-like headaches. Did not respond to 400mg ibuprofen or 10mg Claritin. On day 12 post vaccination, began experiencing photophobia. Mentioned to general practitioner out of an abundance of caution who recommended Excedrin. Took two Excedrin extra strength (total dosage: Acetomenaphin 500mg, Aspirin 500 mg, Caffeine 130 mg). Tension headache ended and most (but not all) migraine-like pain subsided. Some occasional pain ranging from 1-2 out of 10. On day 13 out of an abundance of caution following reports of other events I went to the ER. The doctor there examined for any cognitive symptoms (asked me to follow her finger with my eyes, squeeze fingers, checked ability to move extremities, etc) and concluded no risk of CVST. Determined that vaccine likely lowered my threshold for headaches and migraines (I have had very infrequent cluster headaches lasting for a few seconds in the past, never for this duration). Final diagnosis was migraines. Doctor prescribed Fioricet and gave me a long-acting steroid. Some pain is still there, but symptoms have mostly stopped. Waves of quick bursts of pain on localized areas on the left side of my head are still occuring.

left arm pain beginning 3 days post vaccination; treated at home with OTC pain relievers and active ROM; presented to clinic 2 days after symptom onset; scant ecchymosis noted to vaccination site; site visibly high on deltoid muscle; diagnosed with bursitis and treated with Ibuprofen prescription and rest

The patient was ok when he received vaccine and was advised to stay around pharmacy for 15 minutes. He started to develop red dotty rash in the arms and legs and arms and legs was swollen. was seen by Dermatologist and was give prednisone for 10 days. also had biopsy and the results came back normal. There was no other symptoms other than the rash. (no itching or aches). The rash decreased over period of time and the Dermatologist said it is ok to get the 2nd dose of Moderna and was given 2nd dose on 4/18/2021.

The patient's pediatrician believes the shot was given to high in the deltoid cause shoulder pain. The doctor believes the patient has sirva shoulder.

patient is stating that he is experiencing hair loss after getting the vaccine, he noticed the hair loss more pronounce 12 days after the vaccination

The day after receiving 1st dose, patient experience burning pain at injection site which radiated to chest

Seen 2/22/21 for hives/swelling hands and feet since 2/21 PM (started the day after receiving the vaccine). No respiratory or sytemic symptoms. Treated with Benadryl as needed.

9:59 AM - Vaccine 2:39 PM - Stinging pain in left arm which slowly radiates out from the vaccine site over the next several hours 7:25 PM - Pain reaches base of neck 7:29 PM - Nausea 7:30 PM - Lightheadedness 8:09 PM - Pain reaches tailbone 8:13 PM - Entire back and both shoulders begin throbbing 11:25 PM - nasal congestion 12:15 AM - body temperature drops to 97.9 12:20 AM - shortness of breath, blood oxygen drops to 95% 12:32 AM - blood oxygen drops to 92%. 12:34 AM - blood oxygen rebounds to 95% 12:35 AM - blood oxygen stabilizes at 96%. I am usually at 97-98 while resting. 1:31 AM - one of the worst sore throats I've had in my life begins 2:05 AM - low grade fever 99.1, chills, severe shivering even when under three blankets with heater on full blast, uncontrollable muscle spasms in face, arms, fingers, and legs 7:32 AM - woke up with severe muscle and joint pain. Too bad to sleep through. Can't even walk from my bed to my bedroom door without collapsing. High grade fever 101.7.

Unilateral rash, on right side of face (lower jaw) and neck. Same itchy rash on lower posterior arm (palm facing up). Lasted two days but went away with Benadryl.

Pt developed hives to arms, legs, chest 15 minutes post vaccine. Given Benadryl 50 mg PO with no improvement after 30 minutes. Hives continued to appear and pt c/o headache, left arm pain. Pt taken to ER at medical center. Per mother of pt, he developed SOB, tight chest after arrival at ER and was given Epipen x 2 dose and oral steroids. Admitted overnight for obs and d/c 4/25/21. On 4/26/21, developed hives again and taken with ER.

Black spots on tongue, jaw, inside of lip, red pin-dot rash on knees and elbows, platelet count of 2 (yes TWO), and bruising.

sweating real bad, 102 temperature, bad rash head to toe, eyes swelling shut, lips swelling, headache ER visit on 5/1/21 - hospital gave a steroid and suggested taking Benedryl (have been and continuing to do so) visit with PCP 5/3/21 - prescribed prednisone for 5 days

After the vaccine I had a sore muscle at the vaccination site, but that went away after a day. 5 days after receiving my first Moderna vaccine, I noticed a swollen lymph node under my armpit on the same arm that I received the vaccine on (left armpit), but went to sleep not thinking anything of it. I woke up at 9am the next day to intense body aches and skin sensitivity and the lymph node even larger. The body aches have remained all day, but have subsided from rest. I went to my local urgent care center to get a Covid-19 rapid test and it came back negative.

Started 36 hours after shot, itchiness everywhere and hives accompanied. Redness on face, arms, shoulder, legs, chest, and back.

A day after receiving the vaccine the patient noticed a rash developing on his neck and chest. He went to the urgent care where they gave him prednisone to treat the rash. As of today he states the rash doesn't seem as bad but may be a little larger area than it was. Patient is going to contact his primary physician.

19 y/o male reported back to site after leaving post 15 minute wait period. Pt had redness/rash on L lower arm. Pt had received vaccine at 08:39 in the L deltoid. Pt returned to site at 09:00 with above symptom. Pt states itching as well as slight tightness of the neck with NO difficulty breathing. Pt states felt like a tight short collar. Pt vitals taken and were stable. Rash had progressed up the L arm by time of EMS arrival. EPI was not given in clinic due to no complaint of shortness of breath. Pt care given to EMS crew.

5 days after Moderna Covid Vaccine, patient developed swelling, redness and tenderness over hands/wrists/knees/ankles and feet.

He started with fever and chills on the evening of the day he received the vaccine, Also severe pain in lower sternum, emesis \over the next 2-3 days developed abd pain of lower abd, cough, resp distress, waves of fatigue and hyperventilation, on 5/10 noted full body rash which was itchy, last fever was on 5/13/21, on 5/12 had blood shot eyes, 5/14 - peely skin on palms and face and felt thightness in face

Joints felt sore and stiff, skin felt sensitive, almost like pins and needles- particularly along my lower back.

Patient started to feel dizzy and woozy approximately 30 minutes after the vaccination. Patient states he was having trouble with his vision and focusing on objects. States his feet were really itchy and he was sweating a bit.

Patient stated that he felt itching and numbness all over a few minutes after receiving the Moderna vaccination. He was offered dihpenhydramine but refused because he stated that he had to leave to go to work. He said that he was stopping by his house before work so he would take diphenhydramine if he needed to at home. I told the patient he should wait at the pharmacy longer for observation but he still left because he had to go work. He said that he felt okay before leaving the pharmacy.

Patient received Moderna Covid-19 vaccination at 10:50 AM on June 2, 2021. Within 10 minutes patient describes being shaky. Patient was given orange juice. After 2-3 minutes patient described chest tightness which progressed into left arm pain during our conversation and was not improving. 911 was called. As paramedics arrived, approximately 11:30-11:40 AM, patient began to complain of trouble swallowing but no trouble breathing. Paramedic checked patients heart and patient started to develop hives. The paramedic gave the patient diphenhydramine and then was taken to the hospital for treatment.

Hives and rash all over the body, Angioedema like symptoms (significant swelling of the lips eyes, and face)

NO DRIVING FOR 12 HOURS AND RETURN TO ED IF CONDITIONS WORSEN.

On 05-Jun-2021, the patient received first dose of mRNA-1273 (Moderna COVID-19 Vaccine) (unknown route) 1 dosage form. On 05-Jun-2021, the patient experienced HYPERSENSITIVITY (severe allergic reaction/allergic reaction to the vaccine) (seriousness criteria medically significant and life threatening), PARAESTHESIA (fingers tingling), PARAESTHESIA ORAL (tongue felt abnormally large), URTICARIA (bumps on his face, back, chest and stomach), FEELING ABNORMAL (pattient felt very uncomfortable) and PAIN IN EXTREMITY (Sore arm). On 18-Jun-2021, the patient experienced NASAL CONGESTION (nose was congested), OROPHARYNGEAL DISCOMFORT (throat was scratchy), HEADACHE (really bad headache) and FATIGUE (feeling a lot of fatigue/feeling more tired than he usually is). A few days after the vaccine there was itching all over the body and we went to see the primary care doctor. The doctor was , she gave me medicine called hydrocortisone cream and Prednisone 20 mg tablet. With those medications not making a difference we decided to consult with another primary care doctor who was who gave me the medicine Cephalxin 500 mg capsule, Methylprednisolone 4 mg Dosepk, and told me to take Zyrtec allergy medicine every day. I took the allergy medicine for 15 days. He also suggested going to a dermotologist. So we took that in consideration and when to . She gave me Clobetasol Propionate Lotion 0.05%. All of these did not make a difference so we went to another doctor whose name was who game me Doxycycline Hyclate 100 mg Capsule, Prednisone 10 mg Tablet, and Clobetasol Propionate Ointment. After consuming and using all of these medications I have found no difference than what I had in the beginning, which was more than a month ago.

No Medical History information was reported. On 29-Mar-2021, the patient received first dose of mRNA-1273 (Moderna COVID-19 Vaccine) (unknown route) 1 dosage form. On 29-Mar-2021, the patient experienced LETHARGY (Lethargy) (seriousness criterion medically significant), MYALGIA (Soreness in left arm) (seriousness criterion medically significant), HEADACHE (Headache) (seriousness criterion medically significant) and CHILLS (Chills) (seriousness criterion medically significant). On 02-Apr-2021, the patient experienced URTICARIA (It got progressively worse & got hives) (seriousness criterion medically significant) and PRURITUS (Both arms started itching, mostly the left at first) (seriousness criterion medically significant). The patient was treated with PREDNISONE on 05-Apr-2021 at a dose of 20 milligram and PREDNISONE on 14-Apr-2021 at a dose of 10 milligram. The hives went away after the 4th day but came right back. On 14 Apr 2021, the patient went to the dermatologist. At that time the hives continued and were spread on quads, calves, hands and back. The dermatologist prescribed Prednisone 10mg with titrating dose over two weeks. Pt came to hospital 07/26/2021 for his first COVID-19 vaccine accompanied by his girlfriend. Pt completed appropriate paperwork. LPN reviewed paperwork with pt and gave pt Moderna vaccine Lot# 027B21A expire 09/18/2021 at about 1000 in Right Arm. Pt proceeded to the lobby with girlfriend. At about 1010 pt was witnessed falling from chair in lobby. Staff activated the emergency response protocol crash cart and 02 tank was brought to pt side within a few seconds. Staff called 911. On assessment pt was lying supine in lobby breathing normal and pulse WNL diaphoretic and pale. RN noted that pt eyes where rolling back. Pt head was supported, few seconds later pt came was awake. BP was checked 138/82 in left arm. Breathing WNL and HR WNL. Pt states "I was afraid this was going to happen I had this happen about 5 yrs ago with a booster shot". Pt girlfriend stated that he turned to her and stated I don't feel good and he leaned back into the chair then fell onto the floor bottom first then onto left side then onto back. Pt states he is not hurting anywhere other than where he got his shot. Pt was then assisted to the sitting position on floor. pt stated he felt better just week pt then was assisted to the chair. While sitting in chair pt was diaphoretic and pale pt was given a cold bottle of water and a cool wet paper towel. Pt continued to improve after that. EMS arrived at 1020. EMS got vitals and pt signed EMS document refusing transport to hospital. EMS left at 1027 Pt girlfriend called for a ride because he was driving. Pt back to normal by the time his ride got here at 1043 last vitals WNL BP 126/80.

Intense arm discomfort/pain and injection site soreness, began around 2 to 3 hours after innoculation, lasted around 36 hours. Severe uncomfortable incessant shivering began at 21:00 12/30 and persisted for an hour. Intense cold sweats began at 20:30 12/30 and lasted around 18 hours, this symptom was also accompanied by a fever with a peak of 101.7° that lasted a similar duration. Unpleasant yet bearable body aches manifested the morning of 12/31 around 08:00 and did not subside until the next day. Throbbing, sharp headache began 12/30 at 13:00 and persisted until 12/31 around 19:00. 325mg of enteric coated aspirin was taken on three different occasions, 12/30 at 21:30, 12/31 at 08:30, and 1/1 at 00:30. 1,000mg of acetaminophen was also ingested on 1/1 at 00:30. All symptoms dispersed by noon of 1/1 and have not reoccurred.

Employee recieved the Maderna vaccination at 4pm 1/11/21. Employee was monitored for 30 minutes after vaccination and showed no side effects. He states that at 5pm he had a scratchy throat, at 8pm he had hives and his throat, chest and mouth started to itch. At that time he took Benadry and layed down. He woke up at 10pm with a swollen mouth and throat and was wheezy. He stated he had a hard time breathing and swollowing. At that time he took his EPI PEN. He did not seek medical treatment. Employee states that on 1/12/21 at 9:30am all symptoms had resolved.

L arm injection site immediately with localized swelling and some redness ~ 2 to 3 cm in diameter. No other symptoms or complaints. Monitored x 30 minutes. Swelling and redness resolved after ~ 15 mins.

Low grade fever and moderate chills approx. 12 hours after first dose. Headache the subsequent morning, with side effects gone after that following day, including slight arm pain. Six days later, developed sensitivity to the touch at the vaccine site, which went away after a day.

Moderna COVID-19 Vaccine EAU. About 6 days after the injection my arm is still slightly sore, not uncomfortable but it has also gotten red around the injection site, almost rash like pattern.

Patient received vaccine on 1/13/21; started having headaches on 1/13/21 in the evening. On 1/14/21 developed chills, body aches and nausea. On 1/15/21 had chills, body aches, nausea and fevers as high as 102.0F - also developed lump in left neck on 1/15. Starting on 1/16/21 he noted pain radiating from left deltoid to left hand digits - specifically the 4th digit, palmar surface. Symptoms began improving on 1/17/21 and on that day he mainly felt just soreness in his left arm. On 1/18/21 he noticed the lump in the left neck was still present and he had tightness in the left triceps region.

Rash all over chest, torso, neck, face From 1:00 pm on 1-15-21 and woke up at 8:00 am on 1-16-21 and rash was gone. Took tylenol and benedryl before bed on 1-15-21

Red rash formed up about 4-5 days after the vaccination, the area where the shot was performed had a red circle. Was itching but not a profuse amount.

Fever, chills, nausea, altered sense of taste, migraine, dizziness, nasal congestion, swelling and muscle soreness at administration site Lasted for about 12 hours, only nausea continued for 24 hours Ibuprofen and Naproxen Sodium (NSAIDs) were used from home and provided sufficient treatment of symptoms

starting around 5pm 2/9 there was some soreness at the injection site. if I applied pressure it was very painful. overnight and the next morning the pain got a little worse but its tolerable. the pain increases when i lift my arm. if I keep my arm down by my side there's no pain.

Red and itchy rash around injection site, noticed morning of Feb 12th, as of Feb 15th is still present.

Started with a fever that slowly worsened to a high temperature. This resulted in chills, aches, pains, fatigue. nausea, congestion, sore throat, headaches, and weakness. There was also swelling at the injection site and swelling of my eyelids.

For the first two days a had pain in my left arm but it went away. Around 2pm on that following Sunday (4 days after) I suddenly come down with a fever and headache. I felt so tired, but I took Tylenol and I felt better through the following day. Now, on Tuesday almost a whole week after my shot I get this itching on my chest and arms. Turned out that I got hives, I am not aware of any allergies and I have only broken out into hives one other time in my life and it was 8 years ago and I still do not know the cause. It is now Thursday and the hives have somewhat gone down but I still feel the itchiness around my body. Also to note that the itches are almost about my waist (arms, chest, stomach, back, and neck) I also used the itching cream to relive the itching but I am so tired of the feeling on my body.

Client received 1st dose vaccine and remained in post vaccine monitoring, at the 15 minute point (end of monitoring) before leaving client reported feeling very dizzy with blurred vision. On assessment client was sitting up in chair, alert and oriented, V/S HR 72, RR 20, BP 124/72 O2 sat 98%. Lungs CTA, no reported difficulty with breathing. Client reported additional symptoms of itching, no rash noted and no change in skin color or appearance; nausea was also reported at this time. Information obtained from client on medical history, vaccine history, allergies and current medications. During this time period the itching and nausea was relieved, but the dizziness and blurred vision remained; V/S HR 74, RR 20, BP 128/78 O2 sat 98% . Client reported he started on a new medication Hydroxizine approximately 2 weeks ago and did experience dizziness in the first 2-3 days of starting the medication, but has not had that occur since that time. Client was unable to stand initially for BP check while standing, at 0945 additional medical assistance available to assist: sitting BP 130/76 HR70, standing BP 124/70 HR 70. Client reported no change but states dizziness and blurred vision continued and now he is seeing with blurred vision in what he reported at "cool and warm colors - like a filter on a phone". EMS was called at 0959, arrived at 1011 to evaluate and transport client to the hospital for evaluation. Client departed with EMS on a stretcher at 1014am.

Patient reports routinely having spells of syncope with injections and still insisted on receiving COVID-19 vaccination. Gave injection into left deltoid. Had patient scoot up to table in immunization room with both legs fully under the table just in case there was an issue. I set the timer for 15 minutes and left him in the immunization room with his girlfriend, who also received a vaccine. A few moments later the girlfriend asked for our help stating he was feeling like he was going to pass out. I went into the IMZ room and checked with him. At the time, he was sweaty with a rapid hearbeat. I instructed him to take some deep breaths and went to get a cold paper towel to apply to his head. When I returned he had passed out, not responsive but still breathing, in his chair and his girlfriend was holding his head in her hand. We gently lowered his head to rest on the table while another associate called 911 per girlfriend's request. He began moaning a few moments later and was able to talk to us. He was very hot and sweaty, light headed and breathing rapidly. We applied a cooling pack to his forhead as he continued to lie his head on the table. He then said he might feel more comfortable lying on the floor. We assisted him to the floor, elevated his feet on a chair and placed his head on a roll of paper towels. At that time the paramedics showed up and said his blood pressure was fine and he was just having some hyperventilation. The paramedics got him back up in the chair and his girlfriend got him some apply juice. After about 30 minutes he was talking fine, shaking had stopped and his breathing a temp went back to normal

Headache, chills, fatigue, dehydration, pain at injection site, cough beginning several hours after vaccination and worsening the next morning

8 days after receiving vaccine, pt developed delayed urticarial reaction - evanescent pruritic exanthem on arms and legs and mild angioedema (welts) back of legs.

Swelling, hotness, redness of injection site. In a triangle almost, put Bendaryl gel on it overnight and nothing changed.

about 5 minutes after vaccine pt asked for a bandaid due to some heavy bleeding at injection site. He then asked for water. When I brought the water to him he was very pale and said he felt like fainting. We had him lay down and elevate feet which helped him feel better but was still hot so I applied a cold ice pack on head to help him feel less hot. After about 10-15 minutes he felt better and stayed with us for about 45 mintues until he felt well enough to drive home. I checked on him about 2 hours later and he was feeling much better.

Red blotchy rash about 2 inches in diameter at injection site. Noticed it on day 7 aster vaccine. Not itchy as of now.

Site: Pain at Injection Site-Mild, Systemic: Dizziness / Lightheadness-Mild, Systemic: Hypotension-Mild, Additional Details: pt experienced lightheadedness and blurry vision x 10 min after covid inj which resolved without any intervention

After my second Covid-19 vaccination I immediately felt fine. There was minimal injection site tenderness. It all started to worsen approximately 8 hours after my shot. I lost the capacity to regulate temperature (not to the point of hospitalization), am experiencing muscle fatigue, have severe injection site soreness, and have a massive headache.

General tenderness in injection site day of and day after, nausea and vomiting two days following. Woke up with no pain or discomfort, but as I stood and walked I was in pain from the injection site to my stomach, then I proceeded to vomit and felt fine.

About two days after I received the vaccine, two days later in the morning I woke with hives on the left side of my body.

Arm felt like styrofoam a few hours after injection for a day Severe sore throat started about 5 days after Seen in urgent care 4/23. Hard to talk and swallow

Red splotches around injection site. Not a bumpy rash. Slight itching. About a 2 1/2 to 3 inch area. Moderna COVID?19 Vaccine EUA

Atopic Dermatitis of both elbows (Psoriatic in nature). Onset 1 week post Covid 19 vaccine. Started as small blisters and has now become a thick, scaly rash. Itches and is painful to touch.

Patient has had swelling at site of injection for 10 days. For the last 3 nights he has been having chest pain, back pain and shortness of breath with dizziness.

After 10 minutes after administering the vaccine, the patient fell on the floor dazed. Face was completely red. After a few minutes he was able to respond. Called 911 and the ambulance arrived. The paramedics attended to him. He was able to stand and was assisted to the ambulance for further evaluation.

Per office notes on 3/30/21 from APRN, CNP: About 5?10 minutes after receiving vaccination, patient developed itching to the posterior scalp area and to his lower back. Patient brought back for evaluation. Patient denied any sensations of swelling of the tongue or throat. Denied any wheezing or stridor. Only complaints were itching sensation. peripheral IV started and diphenhydramine 50 mg in addition to methylprednisolone 125 mg given intravenously. Patient had clearing of the hives and red patches after medication administration. Pruritus improved.

I felt fine directly after. But after 8 hours I started noticing hives on my left arm (same arm that received the injection). It was all over my arm from my shoulder to my wrist. The next day I woke up with a weak and achy body and I developed chills and a headache around 3pm. They only lasted that night. I did not take any medication for any of the symptoms. However, the hives lasted 2 more days.

12 minutes after injection - local itching/redness. 30 min. after vaccination anaphylaxis with multiple organ involvement - skin injection site and uvula swelling and eyes itchy. Patient had angioedema of throat with swallowing difficulty. Had mild asthma/wheeze. Also had tachycardia and light headed when trying to sit up. Patient given Epi 0.3 ml IM, Benadryl 50 mg IM, and Pepcid 20 mg PO. Transported to hospital by EMS for 4-6 hour observation. Patient responded well to epi.

Patient reported a sensation of numbness in the vaccine arm for about 3 days after 1st dose. Received 2nd dose today and reported the same sensation starting immediately upon administration. no other symptoms present. Opposite arm from 1st dose. patient walked away before observation period ended.

Post vaccine administration, about 2-3 minutes later, patient reported numbness at injection site and increased warmth radiating to near chest/axillary region. Pt was able to speak clearly, no anaphylaxis. Pt was able to move fingers/arm but numb to injection site. Contacted paramedics who took BP and advised pt to go to hospital - pt declined paramedic escort and volunteered to be driven to hospital by self/guardian

Began night of vaccine, arm was warm , woke up 05/18/2021 L arm was in pain and hot... , later that night he felt nausea.. chest tightening , SOB , pain was intermittent and becoming progressively worse. 05/19/2021 - Underneath his L arm the swelling has increased, with pain and warm. Chest tightness and SOB is intermittent.

Appointment time was 12:00 pm and one hour later symptoms included rash and pain at the point of injection Redness and swelling in the arm Swelling of lymph node under the armpit Nausea and headache The symptoms and pain has continued as of today Redness and swelling started out as the size of a nickel and is now the size of a closed fist Video and pictures have been taken Will be visiting Dr for further assistance today

Patient recevied COVID vaccine and while waiting in lobby became lethargic, clammy, discolored, and was unconcious for about 30 seconds Mother states first vaccine he did not get a reaction just was sleepy. Stated he had Leukemia and has been in remission for the last 10 years.

Received Moderna vaccine on 5/14/21. The morning of 5/15/21 the patient awoke with a pruritic rash on the trunk, which spread to the upper and lower extremities, and face. Patient went to emergency room and was given oral Benadryl x1, oral Prednisone 40mg x1, and sent home the same day with a topical steroid cream. Seen again at my office on 5/28/21. Rash had mostly resolved, but pruritic rash still present on lower back and buttocks. Rash has resulted in small open wounds due to scratching, with surrounding cellulitis. Currently treating with 7-day course of Bactrim for cellulitis. No evidence of anaphylaxis.

patient stated that 1-2 days after immunization their arm become inflamed and almost purple. It was painful and has slowly returned to normal. Some nerve sensations are felt.

2 days after 2nd moderna vaccine started having significant hives - worst over R arm (location of shot) but also over L arm, torso, both thighs. Has now got them recurrently every day for 3 weeks - some days worse than others. responding well to ant-histamines. no GI or pulm symptoms. Is starting to slowly improve but still persistant

"After my first dose, about 24 hours later, I began my onset of symptoms. I developed a low-grade fever, chills, and whole arm soreness around the injection site. After taking off and some Tylenol, my symptoms quickly recovered almost as quickly as they came."

Patient complains of extended experience of pain, swelling, and redness in left unilateral axilla. Pain is significant enough to cause loss of sleep and significant alteration in daily lifestyle. Pain is described as most significant at site of axilla.

Swelling in both feet/ankles, joint pain in both feet/legs, red spots underneath skin in both legs, swelling in left hand

presumptive MIS post vaccine; 2 weeks of headaches, rash, fever to 103, sore throat on 6/26 in ED at hospital platelets 69K; atypical lymphocytoes 0.71; covid PCR neg; ESR 15; CRP 8.6

Rash on arm and chest. It was itching. Started 1-2 days after vaccine. Lasted 1 week after vaccine. Tried cortisone 10 and took plenty of showers to help with itching

Systemic: Allergic: Itch (specify: facial area, extremeties)-Mild, Additional Details: Per pt on left thumb - developed itchy callus on thumb pad that has not resolved. Happened 7 days after vaccine administration, not diagnosised by md

Itchy hives developed on both hands ~24 hours after injection. Most bumps are on his hands and wrist with a few on his elbows.

Sore arm after injection about 12 hours after that lasted a full day or longer. Severe headaches and migraines approximately 2 days after injection. 5-6 Days after vaccine developed swollen lymph nodes in right arm pit and intermitten pain to move arm that lasted for 2-3 days.

Moderna COVID-19 Vaccine EUA adverse event started as localized myalgia around injection site started around 4:30pm same day as the injection then progressed into a headache and fever of 101 by 11pm that night. Myalgia spread to the entire body and interrupted sleep at 2am. Heart rate was 110 while lying in bed. Took Nyquil for fever and at 1pm on 01/08/2021 fever is down to 99.1.

Fever, chills/shivering, headache, nausea, difficulty breathing, myalgia, weakness, restlessness Pain at injection site beginning two hours after injection. All other symptoms experienced during the evening, night, and morning after injection. Sudden onset, lasting 12 hours. Constant. Acetaminophen and ice used to alleviate symptoms.

W/i and hour of injection: Injection site soreness 10 hours post injection: chills, joint aches 15 hours post injection: headache, 101.4 degrees F fever

Patient was seen in my office on 1/15/21 due to a reaction from the covid. Reports that "3 of the reserve doctors were running a covid vaccine clinic and were giving the injections themselves." He reports that he "received the first dose/shot in his left arm then about an hour later received the second dose/shot in his right arm." This happened on Saturday 1/9/21 in the evening and he reports his symptoms started late Sunday night/early Monday morning and were significant by later in the day Monday 1/11/21; he reports he had severe headaches, dizziness, nausea/vomiting, numbness and tingling in both arms from injections sites to forearms, "felt like he was going to die." He is unsure if he had a fever because he did not check it. He reports his symptoms improved on Wednesday and "were pretty much gone by Thursday." At his appointment on with me on Friday 1/15/21 he had no symptoms and his physical exam was normal. No injection site irritation and normal neuro exam. He reports he "notified the reserve doctors about his reaction and symptoms and was told at that time that he was given the vaccine incorrectly because both doses were given at once instead of a few weeks apart." He reports his comrades that also had the vaccine given incorrectly on Saturday "were also really sick" but states that "by Sunday the doctors realized their mistake and were giving the doses correctly just one at a time."

1 rash/blister on finger for about 4 days. Itchy. Skin looks raw. It started out looking like a hive, now it is peeling.

Red circular rash around injection site. Developed ~7-8 days after first shot. Not itchy, sore or sensitive as of 2/2. Has grown slightly since 2/1

Patient reports noticing 10 days after moderna vaccination a bruise like formation on his inner right bicep. He stated that he thought it was a bruise but states that there is no associated pain, burning, or itching. He gradually as time passed noticed many more similar patches present on back of right upper arm into the axillary area. He then noticed more on his back (over both kidneys) and the right groin as well as the inner left thigh and popliteal area. He is noticing more daily. There is no history of eczema . Petechiae looking patches have not changed, according to patient, since first development at 10 days post moderna vaccination. Patient advised that his second dose will be held until he can see a physician. There were concerns about platelet count or possible allergic reaction to first dose. Patient agreeable to terms.

Pt. received his 2nd dose of the Moderna COVID-19 vaccine on 2/3/2021. On 2/10/2021; Pt.wrote an email to staff indicating he had received his vaccine on 2/3/2021 and had since been experiencing "sharp stabbing pain" in his shoulder with movement. Pt indicated in his email the pain felt similar to a rotator cuff injury he had obtained to his other shoulder in high school. Pt. reported he felt like the vaccinator injected the vaccine higher than appropriate which is why he is experiencing pain. RN called Pt to discuss further. Pt. stated the injection site is a yellow bruised color with swelling above it. Pt. states at times he feels like the pain is getting better but then it always seems to return. Pt. denied fever or other symptoms.RN advised Pt. to call his PCP for evaluation as soon as possible. Documentation was reviewed. VAMS did not have the vaccinators name in the system accurately. It sited a vaccinator who was not in clinic on 2/3/2021. Upon investigation from paperwork, Pt.card confirmation, and scheduling assignments the vaccinator was identified as Registered Nurse License # 10.E37725 Exp date: 6/30/2021.

LOCAL REACTION (URTICARIA) AFFECTING LEFT ARM DAY 2 FOLLOWING 1ST DOSE - PRESCRIBED BENADRYL. SYMPTOMS PROGRESSED DAY 3 (21 FEB 2021) DESPITE REPORTED COMPLIANCE WITH BENADRYL TO INCLUDE: URTICARIA AFFECTING TRUNK (CHEST/BACK) AND BILATERAL ARMS/LEGS. NO AIRWAY SYMPTOMS, OROPHARYNX AND MUCOSE MEMBRANES SPARED. VITAL SIGNS STABLE. PRESCRIBED PREDNISONE TAPER; WILL DEFER 2ND DOSE.

About 4 hours after getting vaccine, I started to get a headache and my injection arm started hurting. Then a couple hours after, my skin got sensitive and burning. My muscles started hurting. 10 or 11 hours after getting the vaccine, I got severe chills; migraine; fever; nausea; dizziness. Symptoms lasted through the night. Fever went away 24 hours after receiving vaccine. 14 hours after getting vaccine, headache, arm ache, and body soreness remains.

Patient reports he developed pain and skin blisters on 02/16/2021. He had no other reaction to the first dose. He had chicken pox at age 4 or 5 years old. He had an outpatient visit with a clinician, not at our site. The patient states he was diagnosed with shingles at the left side of dermatomes T11-L3 and was prescribed Valtrex which he was still taking on 3/9/2021. On physical exam that day I observed dry, hyperpigmented pink macules on the left side: 5-10cm clusters just under the umbilicus, hip, lumbar area and buttocks. Gait was normal.

Pain at injection site (began at 6pm 3/12, resolved around 8am 3/15) Fever (began at 11pm 3/12, peaked at 102.0F at 3am 3/13, and resolved at noon 3/14) Fatigue, muscle pains, nausea, chills (began around 3am 3/13, resolved around 5pm 3/14) Chest tightness / difficulty breathing (began 9pm 3/13, reduced intensity on 3/15)

Patient received 1st dose 3/5/21 @15:33 in left deltoid. Reports diarrhea starting within 2 hours on same date that self resolved but reoccurred 3/18 and continues intermittently to date of report. Also reports vomiting beginning 3/9 and lasting through 3/13 but self resolved. Also reports fatigue beginning 3/7 that continues to date of report. Also reports rash at injection site beginning Day 10 post injection that lasted for 2 days. Also reports soreness at injection site from 3/6 - 3/7. Patient denies fever, chills and headache.

Soreness and swelling at injection site for 1 day after both injections. Lasted only for the day following injection. After dose 2, massive headache, fatigue, muscles cramps beginning the morning after vaccination. Most concerning is my heart has been aching and possibly beating out of rhythm. Report submitted night of 2nd day after vaccination. Side effects not subsiding.

Immediately after injection I had soreness and several notable heart palpitations followed by mild dizziness and mild difficulty breathing accompanied with dry mouth. This subsided after a short while (within 15) minutes after I left the vaccine location. I arrived home and felt fatigue and a slight headache develop. The headache has since stopped. I also experienced some chills. No fever as of yet. Soreness at injection site is moderate.

Our Son is typically a happy person and enjoys everything around him and everyone enjoys him, he usually helps with everything involving his care. He uses an IPAD for communication, he understands most direction given if it is something he has learned. Since receiving the second dose of the Moderna vaccination our Son has shown bouts of rage, a rash between his shoulder blades and on his left hip area, both rashes come and go and both rashes are the same type and size. He is typically active; however, the energy level is through the roof, what we mean by through the roof, it is so very extreme that he cannot settle down for anything, he normally sits on our couch and watches a movie but he cannot sit for even 2 minutes without trying to throw himself off the couch. It is typical for him to sleep through the night, but now he cannot sleep more then 3 or 4 hours and he does not go back to sleep, he cries which is not typical.

Patient came in to observation complains of itching of chin, and both arms after vaccination at 3:35pm, initial vitals were taken, and kep monitored, no signs and symptoms of distress or trouble breathing. Advised patient to switch his kn95 to surgical mask. 10 mins after monitoring, patient complains of itchiness of both legs and face. Second set of vitals were taken, no signs of distress and continuous monitoring. Then the patient complains itchy sensation of the tongue and have no difficulty breathing and vital signs are stable and within normal limits. The itchiness did not got better and benadryl IM was administered on his left deltoid , patient was continuously monitored. Patient was informed that for his second dose he may need to go to a medical equipped setting to get his vaccine. No EMS were called. Patient was escorted to take the taxi home. Vital signs remained stable. Nurses informed patient to monitor his condition, and go to seek urgent care if condition get worse. Patient stated he felt stable, comfortable and better when he left the vaccine center.

fever,headache. PT got #2 moderna yesterday @ 1200. c/o headache and fever which started this morning. Pain localized around L eye. No dizziness or visual changes. No SOB. History: pt s/p 2nd Moderna COVID-19 vaccine yesterday and felt like having low-grade temp last night and felt like having hard time breathing deep with some itchy sensation in chest with deep breaths and woke up with fever and headache and muscle aches and took some Tylenol just PTA and feels significantly better currently. No rhinorrhea, cough, chest pain, vomit/diarrhea, rash, sick contact, recent COVID-19 hotspot visit, prior co-morbidities or surgeries.

Patient received Moderna #1 on 3/18/2021 at Hospital. The next day he developed a rash on his face and chest that was not itchy or painful. It lasted less than 24 hours. Then developed dizziness and vertigo for 3-4 days, episodes lasted a few seconds and noted that it happened more when looking at screens, had the sensation of spinning and a feeling like he may fall over. He has also complained of bilateral ear ringing and a dull pain in his left ear.

Patient received vaccine, then immediately described that he had trouble seeing and felt faint. EMS was immediately called. He had difficulty sitting up and kept his eyes closed. He had no reports of difficulty breathing. His body had started to get warmer within 5 minutes. Patient felt better around 10 minutes later when EMS arrived.

The day after receiving his 2nd vaccine the patient began to experience slight itching on the palms of hands. 36 Hours after receiving the vaccine patient noted itching had spread to forearms and was also itching on his lower extremities. At this time there were no noticeable hives. Patient contacted pharmacy 48 hours after receiving vaccination and stated that he now had several hives on his abdomen. Patient was advised to take Benadryl, not to scratch the area, and to keep skin from drying out. Patient was told that if symptoms worsen or if his mouth or throat begin to itch or swell he should call 911 and go to the emergency room.

Patient passed out roughly 5-10 minutes following vaccine while seated on a planter; witnessed by store employee. Hit his head causing a small laceration above the right eyebrow which was cleaned with alcohol and bandaged prior to EMS arrival. The patient experienced dizziness and numbness/tingling in both hands/arms. He was able to tell RPh his name, the year, and knew where he was. Paramedics were called to assess his condition. Per patient and mother, only underlying condition is anxiety and only medication is zoloft.

Weakness, chills, sweating, nausea, dizziness, headache, restlessness, pain around injection site, joint pain, lower back pain

I have like redness, rash in my left arm and right leg (bottom of leg). They are itching . It is like hives I do not know. I used Cetirizin Hexal (antiallergikum) yesterday (13th April 2021) It is continuing, but I do not know maybe it will help.

pt was siting in the post shot area and began to have Urticaria and pruritus symptoms, about 15 min after shot, after another 15 min pt stated he had nausea and a headache, pt sated that he is not doing well. EMS called and pt PCM arrived pt refused trasport by EMS and left with his PCM.

Patient presented to Health Center after receiving COVID-19 vaccine at Health Center, and going back to work site. He stated when he got to work, he began feeling hot, dizzy, nauseous, and pruritic. Vitals stable. He denied chest pain, difficulty breathing. Pt given IM Benadryl 12.5 mg in office and oral hydration w/ water. He was monitored for about 30 minutes, and said pruritus had resolved, but he felt tired. EMS called to transport pt to ED. Pt called by Health Center staff about 2 hours later to see how he was doing. He stated he was on his way home as he had been discharged from ED and told to monitor symptoms.

I woke on the 8th day after the shot with pain around the injection site, extra fatigue, and a migraine. These symptoms are currently ongoing

Oh ok I wrote this earlier but I?ll write it again I got my vaccine at 1:15 I got there early so they have it to me then as opposed to the 2:15 appointment I felt a tightness/ lump in the throat as soon as I got it but I could talk fine and everything was fine as I was taking to people around me I left them came back at 2:15 the person in the pharmacy I had a reaction he gave me a Benadryl and I get fine and he kept me around a bit longer I?m sure he filled the information as he asked for my info and everything then at 4:57 I got some small hives on my right arm not the arm I got it on as I got it on my left arm which went away in 20mins then I went to my local hospital just to be safe and it went away mostly when I got there around 5ish I think? Then they gave me prednisone and reported it as a drug allergy I also got se. wry small hives on Thursday on my hand which went away soon after it came and I?m really hoping I can get a second dose and be fully vaccinated as it was a severe reaction

Patient developed acute onset of hives on face and body and reported facial swelling and difficulty breathing approximately 8 days after receiving first Covid shot. Stated this occurred approx. 11:40pm 3/26/21. Patient arrived at ER 1:10am on 3/27/21 where facial swelling and rash on perioral area, upper back & proximal arms was confirmed. He was given prednisone 60mg, diphenhydramine 50mg & famotidine 20mg. Home RXs for diphenhydramine 50mg po q6h prn, famotidine 20mg po bid & prednisone 40mg po qd provided. He was discharged at 02:48. Patient now recovered but will not receive second dose of Covid vaccine. *NOTE: patient stated grandmother had similar reaction to Covid vaccination which occurred one week after shot.

I have got a bad rash all over my body, it started slowly just on my right arm and shoulder but then spread to the left shoulder/ arm and has been slowly spreading over my neck, forearms, torso, legs, and today it has spread on my face. It hasn?t been that bad but it itches from time to time, it seems to get worse whenever I shower, and now that it spread to my face I look all messed up. I have used an anti itch cream and some aloe to try and alleviate symptoms but they haven?t stopped the rash from spreading.

Patient fainted 10 minutes post vaccine while waiting the 15 minutes in our observation area. He fell out of the chair and hit his head on the floor. He sustained a left frontal abrasion. He was alert and oriented x 3, no neck pain, pale, able to move hands and legs. Ambulated to private car to be taken to the ER for evaluation and r/o concussion.

6 days following 1st dose Moderna, developed chills, myalgia, shortness of breath, injection site arm pain

On 5/5/21 patient developed an erythematous rash with large urticaria, warm to touch on his left upper arm at the site of injection. He has been having intermittent paresthesia of his left upper/lower arm and hand.

Blister at injection site (non-painful) - Noticed this shortly after I submitted my first report. It developed overnight.

I?ve started getting a full body rash. The rash is pretty itchy with some minor swelling but not that bad overall. It?s not at all in my right arm where I got the shot, or in my left arm for that manner. It seems to be nearly entirely concentrated in my stomach region and especially my hands which are somewhat swollen. This reaction is pretty similar to my Penicillin reaction albeit significantly stronger.

Moderna COVID-19 Vaccine EUA. Injection site pain lasted for 3 days. No symptoms for the next 3 days. On the 6th day after the shot, swollen armpit/armpit pain started. Not severe enough to justify seeking medical help. Lapse in arm pain to armpit pain was surprising.

Patient received his Moderna vaccine and immediately (10-15 seconds) appeared to be seizing. His muscles were tensed, tonic clonic, he was pale, hot to the touch, and excessively sweating. However, the fire department reported it as a syncopal episode. The fire department paramedics came and monitored him until his family came to pick him up. His pulse was 57, blood pressure was 102/54, respirations 20, glucose 116, pulse of 96. Patient denied treatment to the hospital and requested to go with his parents. His dad is a physician.

(05/11/21 - second dose administered) sore arm at point of injection. (05/12/21) sore arm at point of injection, fatigue, muscle soreness in upper back, headache, fever, cold chills, nausea, diarrhea. Site: Pain at Injection Site-Mild, Systemic: muscle twitches random, last about 20 seconds. started in arm,moved to other arm, also in neck.-Medium, Systemic: Headache-Mild, Systemic: Nausea-Mild, Additional Details: Patient reports experiencing mild headache and nausea, then a couple days after injection started experiencing muscle twitches started in right arm, then left arm, and today started in neck area. Twitches are random and last about 20 sec. Patient was counseled on hydration and to follow up with medical evaluation if symptoms persisted continued to move to other locations.

Pain and tenderness at injection sight almost imediately. Muscle pain within first hour. Dizziness, weakness, loss of consciousness, nausea, and vomiting approximately 10 hours after injection.

My left testicle began to have random shooting pains that were slightly uncomfortable and after a few weeks of that pain progressively getting much worse and the shooting pains happening more frequently one night my entire scrotum (including my right testicle) began to hurt so bad and for such long periods I went to the emergency room. I had an ultrasound done as well as tests for STDs and STIs and had no structural damage in my testicles nor any sexually transmitted problems. It has gotten somewhat better but there is still some general discomfort in my scrotum. While it was at its worse my scrotum was somewhat swollen (which it still is slightly), very red, and looked as if there were numerous blood clots on the surface of my scrotum.

Headache arm where she recieved injection is sore. Narrative: Employee awoke with headache this morning at 0430. Employee has not taken any OTC meds for headache. Employee was instructed to take OTC acetaminophen/ibuprofen.

He reports that the same evening of the vaccine he noticed arm swelling, pain at the site and generalized body aches. He also reports that his tongue and throat felt numb for four hours. He also noticed a little cough, shortness of breath and a runny/stuffy nose.

Patient presented with malaise, dizziness. Then has rash on back, given Benadryl 50mg PO. 15 minutes later c/o throat tightening and was given epinepherine 1:1000 IM. He was watched closely for 2 hours and VSS. He was then released to his grand father at 11:38 and to home

Headache. Pain at the injection site. Ache's all over and slight nausea. I also have a low fever of 100.9.

Today (9 days after injection) the marked area has not gotten any larger. It is not as red or warm to touch and no itching. Marked area is still "pink"

Doctor diagnosed me with pityriasis rosea this week, and the rash has been there since January 24th, 20 days after my first vaccination dose. The herald patch of pityriasis rosea appears 1-20 days before the rash, and I don't know when it appeared since it was not itchy and on my back, but I did have some mild symptoms such as a mild sore throat that can be early signs of this disease.

22 yo M w/ no known chronic medical conditions, got COVAX in L delt 1 week ago, now w/ raised/erythematous/itchy rash overlying L delt that started last night. The story sounds c/w localized vaccine reaction, but the area is indurated/warm, so just in case of cellulitis I plan to treat w/ keflex. I do think there is an inflammatory/allergic component, so I also prescribed topical hydrocortisone for itching. Discussed plan/SRP w/ pt in detail. Answered all questions. Pt verbalized understanding, agreed to plan/SRP, and pt was d/c'ed in stable condition.

Moderna COVID-19 Vaccine EUA The patient was in his usual state of health until he received his second dose of the Moderna COVID-19 vaccine on Tuesday 2/23. He had previously had no side effects from the first dose other than soreness at the injection site. After the 2nd dose, he noticed arm soreness at the injection site more quickly than before. 10-12 hours later, he noticed headache and subjective fever, which was relieved by Tylenol. On Wednesday he woke up with subjective fever and chills, and felt sternal chest pain during deep breathing and attempted cough, but did not notice any effect from positional changes. Other than attempting to cough to elicit pain, he did not experience any other symptoms such as runny nose, coughing, sneezing, congestion, shortness of breath, palpitations, abdominal pain, diarrhea, or dysuria. The following day, he noticed that the chest pain was more severe and persistent throughout the day. The pain location would vary from the sternal area to the left side of his chest. The intensity of pain would vary between a 2-3/10 to 7-8/10. When the pain peaked, it would be accompanied by feelings of fever, chills, and brief headache, and he felt that the pain would "spread" across his chest. The pain would persist for a variable amount of time but then eventually disappear before returning. The pain was not affected by deep breath, position, or analgesics (he took an ibuprofen at around 2 AM when he couldn't sleep). After he fell asleep he was not woken up by pain. On Friday, he pain continued to be intermittent, and peaked during a work meeting. At that point he told his boss and an MD co-worker, who recommended that he presented to the ED. The EKG that was obtained in triage showed sinus tachycardia widespread ST elevations, PR depressions, with reciprocal changes in aVR. At this time he was continuing to experience chest pain. After getting a room in the ED, he no longer felt pain. He received a 2nd EKG showing improvement in ST elevations in some, but no all leads. He received ibuprofen and he continued to feel no symptoms. He was found to have a troponin elevation of 8.84 that peaked to 11.2. Transthoracic echo showed no motional wall abnormalities, normal EF, no pericardial effusion. He was admitted to the CICU out of concern and workup for myopericarditis.

I was not the vaccinator. Pt came to office 3/9/2021 for a rash. Only thing different is that he received Moderna Vaccine

24hrs after his vaccine got hives on his arms and legs, it lasted around 12hrs and resolved on its own. Had some fatigue aferwords and some sensation of low grade fever those resolved within another 12hrs. Denies any other symptoms or concerns. He thinks he had covid in December. His friend dx with covid and they were together w/o masks and he got respiratory symptoms but did not get a confirmatory test.

On Thursday, March 11th, 2021 approximately 3:10 pm, Patient received his first dose of Moderna Vaccine (Lot number: 003A21A) in his left deltoid Patient received the Moderna Vaccine. After which he sat in the designated medical observation area for 15 minutes. Approximately five minutes later around 3:15 pm, Patient experienced syncope, fell out of his chair, hit the left side of his forehead on the ground. Due to the fall, he experienced a mild abrasion on the left side of his forehead, which led to bleeding. The Physician?s Assistant rushed to aid Patient who was lying on the ground in a supine position. Emergency Medical Services (EMS) - Fire Department and ambulance were called immediately and approximately five minutes later EMS arrived. During the time frame prior to EMS arrival, the Physician?s Assistant asked numerous questions to Patient to assess his condition. She asked Patient if he could breathe and he responded ?yes?. She asked Patient if he was short of breath and he responded ?no?. She asked Patient if he felt like his throat was closing and he responded ?no?. At this point, an Epinephrine pen was not used. Patient was alert and the Orient x 4 questions were asked [(1) Who are you? (2) Where are you? (3) What is the date and time? (4) What just happened to you?]. Patient also denied head pain, neck pain, nausea or chest pain. Staff performed a limited neuro-exam and was deemed within normal limits. Patients Glasgow Coma Scale score was 15 Patient mentioned that he has a phobia of needles and something like this has happened before when he was ?young?. Patient initially did not want an ambulance to be called. It was explained to him that it might be in his best interest to call EMS due to syncope and abrasion on his forehead due to the fall. Patient also mentioned that he only consumed two cookies and coffee for that day. When the EMS arrived (about 5 firefighters), they interviewed him and performed a blood pressure test. His sitting blood pressure was 130/82. One of the firefighters assessed his spine and Patient denied cervical, thoracic, and lumbar tenderness or pain. The firefighter assessed Patient's glucose level, which was a bit difficult to obtain in the first initial efforts, with the glucometer reading ?error?. The firefighters eventually were able to obtain a reading on Patient's glucose and it read 86. Patient was able to stand on his own, and a second blood pressure test was performed; his standing blood pressure reading was 145/89. EMS gave Patient gauze to address his head wound Patient initially refused to go to the hospital, but per EMS protocol, fluids needed to be administered. Patient entered the ambulance and was taken to the hospital by EMS.

Extreme light-headedness and pallor. Subsided within 15 minutes. The following day: injection site soreness and general fatigue.

Woke up around 3 am with a mild headache, soreness, and mild fever. Went back to sleep and woke up again around 7 am. Had mild fever and soreness throughout the day, but all symptoms subsided by the end of the day. Slight soreness at the vaccination site persisted for an additional day before subsiding.

1 hour after, experienced mild soreness around site of inject and elbow of injection arm Ramped up until 4 hours later it was very sore at the shoulder, especially when rotating the shoulder back or lifting it. Pain continued until 9pm day two, when I took a tylenol for relief, along with pain. Around 5pm day 2, fatigue, chills, sore throat, and mild 99 fever began and continued until the tylenol at 9 pm. Day 3, woke up with slight arm pain and very mild cold symptoms, including fatigue and head congestion.

"Moderna Arm"; Localized red rash near injection site that appeared around 7 days after injection. Warm to the touch but little to no itching. Slight underarm lymph node tenderness. Rash approximately 1 inch in diameter.

A spontaneous report was received from a Consumer concerning a 22 years old male patient who received Moderna's COVID-19 vaccine (mRNA-1273) and experienced cellulitis, skin felt hot/feeling hot, arm was red/redness generalized, arm was swollen/peripheral swelling. The patient's medical history was not provided. No relevant concomitant medications were reported. On 14 Jan 2021, prior to the onset of the events, the patient received their first of two planned doses of mRNA-1273 (lot/batch: 012L20A) intramuscularly in the left arm for prophylaxis of COVID-19 infection. On 15 Jan 2021, the patient experienced the events as cellulitis, skin felt hot, arm was red, arm was swollen. Treatment information was provided as patient has taken antibiotic Cephalexin 1 capsule four times a day for a week. Action taken with mRNA-1273 in response to the events was unknown. The seriousness criteria of event cellulitis was assessed as serious as per EMA list. The outcome of the event swollen arm was resolved. The outcome of the events cellulitis, skin felt hot, arm was red was unknown.; Reporter's Comments: Based on the current available information and temporal association between the use of the product and the start date of the events, a causal relationship cannot be excluded.

Patient with severe dizziness and intractable nausea/vomiting. Upon admission to the ED last night his platelet count was 9. He has a subconjunctival hemorrhage OS and frank hematuria, besides the epistaxis. Pinpoint erythematous lesions noted on his forearms. No other bleeding noted. ADAMTS13 level confirms TTP . Patient now on day 4 of PLEX treatment.

Fatigue began around 12:30 pm, around 3 hours after shot and increased the next day. Pain around injection lasted about 3 days. Muscle aches and joint pain started on second day and continued mildly on third day. Chills experienced on second day, no fever.

after receiving the 1st dose of Moderna covid-19 vaccine, while patient is sitting for observation, patient started to feel itchy, I grabbed Benadryl syrup to administer by patient stated he feel he is going to faint and started. I supported him to lay him on the ground, while laying him on the ground I called for associate nearby for code to call for help and I grabbed Epi-pen and I was about to administer. During this time patient was still responsive and did not lose conscious. Before I administered the Epi-Pen, patient stated he was doing ok, he did not lose conscious so I did not administer the Epi-pen. another pharmacist applied a cold compress as patient was sweating. after few minutes laying on the floor patient said he was feeling better so we put him in a chair. while being seated in the chair patient stated he feels his throat is tightening and I administered 10 ml of OTC Benadryl Syrup. after few minutes patient stated his throat is getting better. Patient remained seated for 30 minutes for observation and kept applying cold compresses. After 30 minutes, patient stated he was doing better, I advised him to contact his physician and he should get the second dose after consulting with his physician and he needs to take it in a medical office

102 degree fever developed Saturday night (2 days after vaccination) and has continued through Tuesday morning (5 days after vaccination), along with nausea and intermittent chills. Use of both ibuprofen and acetaminophen reduce the fever to about 100 degrees. Injection site pain began the day of vaccination but subsided after 2 days.

Around 3 pm on 4/14/21, I started to feel very tired and I started to get a headache. Around 6 pm, I started to experience lower back pain and my legs began to throb and my whole body became very sensitive to touch. I took tylenol or ibuprofen every 4 hours for the rest of the night. This went on until Thursday 4/15/21, when the dizziness and throbbing headache started. Every time I would stand up I would become off balanced and need to hold on to something. I was still experiencing the lower back pain, leg throbbing and skin sensitivity but the dizziness and headache became much more intense. Around 9:30 pm on Thursday, I felt like I was hallucinating and was experiencing euphoria along with a fever. I did not sleep the night of 4/15 into 4/16. I was sweating profusely with a throbbing headache that would cause spinning when I would stand up. Around 3 am on 4/16, I fell asleep until around 4:30 am when I woke up in a pool of my own sweat. The body aches, leg pain and skin sensitivity went away along with my fever. Throughout Friday 4/16, I had a throbbing headache and was dizzy with no relief. This continued into Sunday 4/18 when I started to feel some relief. My headache was pretty much gone but the dizziness is still very much there. I have also an experienced elevated heart rate and respirations throughout the last 5 days.

Don't remember hitting horn to alert officials. Got out of car, dizzy and stumbled. Blacked out and fell to pavement. Don't remember falling, woke up with extreme mind cloudiness, drowsiness, and hands were numb to touch but felt extreme pressure on palms. When pressure subsided, finger tips felt inflamed.

AFTER 10 HOURS OF ADMINISTRATION OBSERVED SEVERE ARM CONTRACTURE - 15 MINUTES OF BOTH ENTIRE ARM STIFFNESS AND FINGER CURL LASTING AT LEAST 15 MINUTES. PT LOST SENSATION IN HIS ARMS UP TO HIS ELBOWS AND BOTH HANDS. HE ALSO SUFFERED ARM SORNESS AT THE INJECTION SITE, HEADACHES, FATIGUE, VOMITING AND JOINT PAIN, AND RASH AROUND HIS EYES. UPON HIS REPORT TO ME A FEW HOURS LATER HE HAS RESUMED SENSATION IN HIS ARMS AND IS FEELING SOMEWHAT BETTER NOW.

Major depressive episode, severe arm soreness at injection site, redness and swelling at injection site, fever, chills, fatigue, headache, muscle aches.

Client reported feeling itchy on his arm and felt that his throat was itchy to EMT in observation. EMT did assessment. ABC's intact. No rash noted by EMT on assessment. Patient stated he had a history of anaphylaxis but doesn't feel the same as it did when that happened. 50 mg of benadryl administered to client.

The patient has had pain redness and swelling at the site of injection for the past week. on Day 5 he developed a diffuse rash throughout his chest abd and back. It is ichy and a bit uncomfortable.

On May 04, 2021 this 22-year-old male patient received his second COVID 19 vaccine on his right arm at 1705. On May 04, 2021 at 1755 the patient complained of pain at the injection site and was treated with acetaminophen 650 mg, the patient also requested acetaminophen on May 05, 2021 at 08:34, 14;49, and 20:50. On May 07, 2021, I spoke to the provider who stated the patient has not complained of pain from the injection site

Presented with left neck, chest and upper extremity weakness, pain and burning that started on 5/1. Stroke protocol followed and alteplase given 5/2. By morning of 5/3 all symptoms had resolved.

Pt felt nauseous, hot, started getting red, and sat down on floor about 5 minutes after receiving the vaccine. I was alerted and I helped him into the vaccination room to lay down on the floor with his feet propped up. His girlfriend was there to help calm him down. I offered to call 911, he refused. I grabbed a fan for him, and turned it off when he requested. He then began shivering rather uncontrollably and said his left arm (injection site) was hurting and tingling, and at one point, said his right arm was beginning to tingle as well. We had him breath and relax, he began to return to normal and he and girlfriend discussed how he must be partly having an anxiety/panic attack. He trialed standing up a few times until he felt good enough to leave the room, all in all about 20 minutes. He sat close by for additional 15 minutes of observation, at which point he left stating he felt more like himself.

I received my second dose of my moderna COVID-19 vaccine on Sunday, May 9th. On May 13th around 2:40 pm, I began to experience a full sensation in my right ear. My hearing felt slightly muffled, with mild ringing. It has been 5 days since my symptoms started. On May 17th I went to my primary care provider and was told I had redness and inflammation inside my ear. I was prescribed sterile otic suspension drops of Ciprofloxacin 0.3 % and Dexamethasone 0.1% for use of 7 days. This is my second day of treatment, however I feel the same.

High fever approximately 12 hours after injection of second shot and drowsiness approximately 4 hours after injection. During the fever, had significant trouble walking and moving any part of body. Significant arm soreness at site of injection. All symptoms resolved themselves within 24 hours of receiving the vaccine aside from the arm soreness which resolved itself within 72 hours.

Within 10 minutes of receiving vaccine, the patient became hot and sweaty, felt like pins and needles sticking and itching all over, felt as if he couldn't breathe. BP 184/143. He then began projectile vomiting. I administered and epipen (one dose) and had pharmacy staff call 911. He began feeling better within a couple of minutes, and by the time EMT's arrived, his BP back down to 118/73 and he stated he felt fine. They transported him to the ER. I called patient approx 6 hours later. He stated that he was home without further treatment from the ER and felt fine.

Rash on hands that started one week after vaccine, It is mildly itchy, localized essentially to the hands now but he also had a few spots in his feet, has never had any sores in his mouth, he has no new soaps, products, detergents, he describes it is mildly itchy like a mild case of poison ivy, there has been no breathing problems, he is not having to use his rescue inhaler more than average. He is using Benadryl and over-the-counter hydrocortisone cream which seems to indicate his symptoms his main question is should he proceed with his next vaccine dose which is scheduled for 05/20/2021. He has had no fevers, chills, nausea, vomiting, diarrhea.

Pt woke up this morning with hives on his torso and head, and his head was itchy. He also woke up with the normal fever reaction to the Moderna shot. Pt was instructed to take an antihistamine by a family friend nurse, so he took loratadine 10mg tablet and has felt fine since.

Pt refused to go to the hospital, he left with his wife and she drove. I told the patient I would follow up with him via a phone call to see how he was feeling. I called pt at 5 pm on 5-28, he stated he was fine

Hepatitis infectious mononucleosis; tired; Sore Left Injection site arm; This spontaneous case was reported by a consumer and describes the occurrence of HEPATITIS INFECTIOUS MONONUCLEOSIS (Hepatitis infectious mononucleosis) in a 22-year-old male patient who received mRNA-1273 (Moderna COVID-19 Vaccine) for COVID-19 vaccination. The occurrence of additional non-serious events is detailed below. No medical history reported. Concomitant products included LEVOCETIRIZINE DIHYDROCHLORIDE (XYZAL) for Allergy, IBUPROFEN for an unknown indication. On 05-May-2021, the patient received second dose of mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) 1 dosage form. On 05-May-2021, the patient experienced VACCINATION SITE PAIN (Sore Left Injection site arm). On 06-May-2021, the patient experienced FATIGUE (tired). On 17-May-2021, the patient experienced HEPATITIS INFECTIOUS MONONUCLEOSIS (Hepatitis infectious mononucleosis) (seriousness criterion medically significant). The patient was treated with AZITHROMYCIN ongoing since an unknown date at an unspecified dose and frequency and AMOXICILLIN ongoing since an unknown date at an unspecified dose and frequency. At the time of the report, HEPATITIS INFECTIOUS MONONUCLEOSIS (Hepatitis infectious mononucleosis), VACCINATION SITE PAIN (Sore Left Injection site arm) and FATIGUE (tired) outcome was unknown. DIAGNOSTIC RESULTS (normal ranges are provided in parenthesis if available): On 12-May-2021, Alanine aminotransferase: 620 (Inconclusive) 620. Got the shot at 12:30PM on Thursday. Started feeling fever, chills, fatigue, muscle and joint pain, and slight headache at 3:10AM. Recorded temperature of 100.7 degrees with an ear thermometer. Took Tylenol, drank 32oz of cold water, put a cold wash cloth on forehead. After an hour my temperature dropped to 98.8. Felt less achy but still warm.

Woke up next morning with hard lump around deltoid along with, red burning rash from injection site radiating down to elbow. Rash along biceps and lateral upper arm. Stinging sensation when arm is bumped or upon touching. Injection site was red, hot and hard the next morning.

sudden hearing loss requiring hear aids, ear canal swelling and itching, axillary gland enlargement and pain

CHIEF COMPLAINT & HPI: 22-year-old male here with left shoulder pain at the site of his second moderna vaccine which he received yesterday. patient with subjective fevers and chills. no redness or swelling. Also some axillary lymphadenopathy. No numbness or weakness.

I received the vaccine through my employer. 7 Days after the vaccination event, my left deltoid muscle became significantly swollen, stiff and tender. There was pain when I tried to elevate the arm laterally. The site was very warm to the touch. I experienced periods of malaise and fatigue at random intervals throughout the week after which I received the vaccine. On day 8, a red rash began to appear on the vaccination site. It was slightly itchy. The area was still stiff and swollen, and painful to move past a certain angle. This night I felt especially fatigued. On day 9 I woke up to see the swelling was mostly subsided, however the rash had spread down the arm to roughly the elbow. The site was no longer warm. It was still slightly irritable. As the day is progressing (this is written on day 9), the rash is starting to fade.

Approximately 10 minutes after receiving vaccine patient began to experience some mild discomfort on the left neck (mild "stiff neck"), heavy arm or discomfort, and mild tingling in the fingers. Initial BP 149/92 HR 91. Patient clinically stable at 30 minutes post-vaccination. Neck pain gone 30 minutes post-vaccination; patient only feels pain at injection site. BP 143/95 HR 86 O2=98%. Patient released from clinic at 2:22pm.

Early on in the day of 1/6/21, I felt very sluggish and spaced. My boss asked me early in the day if I was okay because she said that I seemed "out of it." It wasn't to the point that I was unable to work, but it was a noticeable difference from my typical state. At 11:31 AM, I texted my wife and said "I feel gross and weird today." When she asked me how so, I replied, "I just feel run down, my neck hurts, and I can't focus." My arm was more sore than it typically is from the flu shot, but it wasn't unbearable. As the day went on, I felt the cloudiness in my head start to dissipate a bit and I started to feel a little better. By dinnertime, I was feeling sick to my stomach and was not able to eat very much. I chose to head to bed around 9 because I was feeling pretty ill (my neck had really started feeling sore again and my stomach was very upset.) At 1:30 AM on 1/8/21, I woke up with a horrible headache. I get migraines from time to time, but this was unlike anything I had ever felt before. It felt like my head was in a vise and I was very close to asking my wife to take me to the ER. I never go to the doctor, but it legitimately scared me. My neck was extremely sore and my head hurt so bad that even laying it on a pillow hurt. I was extremely nauseous as well. I took a few extra strength Tylenol and laid my neck on a heating pad with an ice back on my forehead. The sound of our humidifier was too much for me, so I also out in earplugs. After an hour or so, I fell back asleep. When I woke up this morning, I still had a headache and I took an Excedrin. To this point, I have felt a little sick to my stomach, but the intense headache has been kept at bay. My neck is still sore but it is also somewhat better.

Day of injection: weakness and site pain Next day: Fever, skin hot but shivering cold (chills), headache, body aches Day 2: same as next day but less severe

Small amount of hives, took an allergy relief and symptoms went away. Hives came back, took Benadryl and symptoms went away. Repeat process for 4-5 days. On 5th night, increase in amount of hives, all over back and face, took Benadryl and eventually went away. Went to walk-in, provider recommended Claritin and Pepcid daily. I've taken those for three days now with the occasional Benadryl and haven't noticed any hives. Hives varied in size and location. Ultimately in all different areas of my body at one point of another. I have pictures if needed.

Patient reports receiving Moderna vaccine 6 days ago. Felt fine for first 30 minutes following vaccine, but developed migraine headache next day, which resolved after 24 hours. Reports developing pruritic urticarial rash and swelling to hands and feet yesterday. Denies facial or mouth swelling. No chest tightness, wheezing or SOB. Denies previous reaction to vaccines. NKDA. Took benadyl yesterday and today, which did help. Assessment and Plan: 1. Urticaria - Possible correlation to Moderna COVID-19 vaccine received 6 days ago. Will treat with antihistamines and topical steroid cream. Avoiding systemic steroids, as do not want to suppress immune response to recent COVID-19 vaccine. - Treatment with antihistamine- Benadryl OTC and Claritin. - Provided patient with V-safe smartphone tool info to report potential side effects of COVID-19 vaccine. - Completed VAERS report for potential reaction. - Advised employee patient also follow-up with occupational health regarding plans for second vaccine dose. - Follow up at ED if facial swelling, SOB or if symptoms worsen. - Will route chart to PCP. - TRIAMCINOLONE ACETONIDE 0.1 % TOPICAL CREAM APRN.CNP  

noticed a red cirlce around the area where I received the vaccine. Does not hurt, no itch, no swelling. Just there and really red after shower for example. Notice it when I work out as well. It is more red when I am working out. The shot was injected on my left arm.

Within 15 minutes of receiving vaccine patient was noted to have hives/redness to face, neck, chest, back and upper arms. He was given 25mg of liquid benadryl. BP was slightly elevated at 140s/90s with pulse between 100 and 110. He was observed for 2 hours and then driven home by family.

Headache, lasting over 12 hours. No medication taken. Nausea, for 2 hours on the night of the 21st Vomited once the morning of the 22nd. Injection site pain, started several hours after administration, ongoing 16 hours later. High Temperature. 101.3 the morning of the 22nd. 103.0 at 10:00am

8pm: Headache, pain at injection site 3am: Woke up with extreme chills, severe night sweats and fever of 10.5 7am: Chills, fever, severe pain when moving arm at injection site 9am: Chills subsided, severe migraine, and fatigue, low grade fever 3pm: All symptoms subsided except for pain at injection site and headache

Patient received Covid-19 Moderna vaccine @ 0750, no adverse reaction noted after 15 min of receiving vaccine. Patient reported back to work & noticed Right upper chest/shoulder to mid chest on opposite side of where vaccine was given (L deltoid) flat, red rash w/ 7 small bumps ?hives w/ some itching. No difficulty swallowing, wheezing, SOB or tightness in chest Page to Allergy team, verbal order for 10mg PO cetirizine, no further workup needed and patient can return back to work

Soreness in arm at injection sight. Muscular pain and general fogginess of thinking for the next 24 hours with intermittent migraines.

Red, itchy rash on left arm at approximate location of injection site, starting 1/28. Still present 1/30/21. No other symptoms reported.

Woke up with a headache and fatigue, as well as soreness near area of injection. Energy was very low and felt very tired. Tried to sleep at around 12:00 PM but had a thumping in my head that would not let me fall asleep. Around 3:00 PM tried to sleep again but being shivering/shaking a lot despite being under a blanket and comforter. Eventually needed to take a hot bath for around 45 minutes to an hour. Took my temperature while in the bath which read 96°F (although that thermometer may not be the most accurate.) After the bath I went directly to bed with a comforter, blanket and weighted blanket and was able to fall asleep for about 90 minutes. Was no longer shivering when I awoke and felt much better. Went to sleep at around 9:00 PM and felt fine when I woke up the next morning.

Itchy skin accompanied with small red bumps, mostly on hands and arms, bilateral, not near injection site. Minor discomfort, treatment not initiated.

On day 10 post vaccination, patient started to develop hives on hands, elbows, knees and feet. They have recurred each day since (total of 4 days so far). Patient has been taking loratadine each day since appearance and using hydrocortisone cream but has not yet sought any medical visit.

Moderna COVID-19 VACCINE Fever, chills, cough, sweating overnight and then loss of taste and smell the next morning. Soreness at the site of the injection. Extremely fatigued. Tmax 100.5 Bodyaches, Headaches , Nasal congestion, Shortness of Breath, Chest Pain. Four days after 2nd Moderna vaccination his loss of taste and smell is still affected. His left arm soreness still persists. Bodyaches and fatigue still persists. Headaches comes and go. Nasal congestion persists Resolved Shortness of breath,Chest Pain, Fever and cough

2/10/21 at 9:30 pm, the night after I received the vaccine, I noticed a red rash on my chest/stomach. The next day I noticed that it had spread a bit to my back. As of right now, 2/12/21, it has lightened to a more pink color. It doesn't hurt, at most it itches but otherwise I don't really notice it.

I have a big red rash on the front side of my neck. It?s really itchy and dry. When I put on lotion, anti-itch cream, or soap when I shower it stings really bad. I though the allergy could have been other stuff but I can?t think of anything else. So, it has to be the vaccine.

Really sore arm beginning around 2 hours post injection, worst at second day, resolved after fourth day.

Patient reported itching all over a few minutes after receiving vaccine. Patient said he felt fine other than that. I advised patient to take Benadryl.

Nothing at time of injection. In the evening, soreness in the arm (strong but on par for what I would expect). Next morning, I starting feeling sick, with flu symptoms. Aching muscles, sensitive skin, mild headache, sore eyes, and in the evening, nausea. Temperature gradually rose. ~99.7 at 6pm, 100.1 at 7pm, 100.7 at 8pm, 101.7 at 9:30pm. I took some Tylenol around 7pm, and then some theraflu at 10pm. Fell asleep at 11pm. Periodically waking up every 30min from 1am onwards. Muscle discomfort, sweating, and nausea continued until 3:45 AM, at which point I was feeling better and was able to sleep continuously. Feeling much better in the morning on March 4th. Temperature was 99.9, then rose to 100.0 at 11am. Now, at 3pm, feeling fatigue, exhaustion. No headache. Normal sense of taste and smell. No couch. Taking another day off from work. Temperature is currently 100.0. Taking more theraflu now, have not taken Tylenol today.

Patient received a COVID-19 vaccine on 04MAR21 @ 1400. At 1645, he returned to the vaccine room shaking and his eyes were tearing up. He complained of pain at the injection site, headache, nausea, abdominal pain, chills and joint pain. His temperature was 98.4 @ 1712. He also complained of increased saliva production. He laid down on a cot and an emergency rescue blanket was applied. He was reassured and monitored by our onsite RN. He was stable and left at 1755.

Service member received Moderna Vaccine Dose 1 on 01 FEB 2021. Service member experienced skin irritation, redness, and hives within an hour of receiving the vaccine. After going back to work, SM's symptoms worsened with itchiness and throat soreness. Symptoms progressed at home and noticed hives all over body and swelling around the face. Patient saw a provider in Clinic. He was prescribed Prednisone and Zyrtec which provided relief and symptoms improved and were mostly gone by Day 4. Instructed by his commander and provider to refrain from getting second dose of the vaccine.

Shortly after receiving the second dose, I began experiencing some soreness around the injection site in my left arm. This worsening throughout the day, and around 5:30pm that day I began to experience some cold-like symptoms (fatigue, soreness, chills). This continued into Sunday, and it tapered off until I felt relatively normal around 4pm. I only treated this with some generic Nyquil.

Diffuse blanching maculopapular rash on face, chest, neck, arms, and palms of hands. 9cm (L)x6cm (W) red patch on left upper arm near injection site.

pt initially developed hives the day after the vaccine, but on 3/22 he developed left groin pain. exam suggestive of epididymitis with no other known precipitating event other than recent vaccine

10 days after my first dose I have a ~3.5" diameter oval red spot on my arm at the spot of injection. It is slightly itchy and a little tender to the touch. Not painful though.

covid vaccine sickness. presents with chills, dizziness, n/, coughing, sore and tenderness on injection site a/f 2nd Moderna on 6 Apr.

PATIENT STATED HE WAS FEELING REALLY HOT AND DIZZY. PATIENT WAS HOT TO TOUCH. PATIENT THEN STATED HE WAS HAVING DIFFICULTY BREATHING. I HAD HIM LAY DOWN AND ELEVATED HIS FEET. KEPT TALKING TO HIM AND ASKED HIM IF HE FELT LIKE HIS BREATHING WAS GETTING WORSE AND HE SAID YES. I ASKED TO CALL 911 AND I ADMINISTERED ONE DOSE OF AN EPI PEN INTO HIS RIGHT THIGH. WITHIN 30 SECONDS I ASKED HIM HOW HE WAS FEELING AND HE SAID HE COULD BREATH BETTER, HIS SKIN STARTED FEELING COOLER TO THE TOUCH. PARAMEDICS ARRIVED AND TOOK HIM ON TO THE EMERGENCY ROOM.

A hard, tender lump/rash formed on my left arm where the injection was administered. When it first formed it was itchy but that subsided by the following morning. It developed 7 days after the injection. There have been no other side effects.

Arm slightly sore about 2 hours after injection. Only upper arm near injection site was sore for a little over 48 hours. Headache and fatigue started about 9hours after injection. Continued symptoms lasted about 48 hours.

On 4/15 at 1:50pm my injection arm started getting very itchy. I have a sore located 3 inches next to the injection site. Approximately 1 minute after it began to itch I got very hot and nauseous, approximately 30 seconds later I was unconscious and beginning to seize. I was unconscious for about 1 minute. When I awoke I couldn?t see anything, my entire body was numb and in pain. I couldn?t move my fingers. I was rushed to the ER but by the time I was seen I was fully coherent and fine.

Starting about 10 hours after receiving the second Modern vaccination I experienced a minor headache and drowsiness, which continued until about 30 hours after the vaccination. These symptoms resolved without treatment. Around 20 hours after the vaccination an itchy rash developed on the lower lip, around the left eye, and in the genital area (including: penis, testicles, and upper inner thigh). Rash continued to get worse until about 4 days after the vaccination. At this point I sought treatment at a local urgent care. The urgent care personnel said they believed the rash symptoms were caused by the Modern COVID-19 vaccination, did not identify any other possible causes, and encouraged me to report it to the VAERS system. The treatment followed was topical 1% hydrocortisone cream applied to the affected areas, and 12.5 mg of diphenhydramine taken orally before bed. The rash symptoms improved with treatment but have not fully resolved at 5 days after the vaccination.

The patient received his first Moderna Covid-19 vaccine about 3:30pm. At about 3:45pm the patient presented with generalized itching. He stated that he had taken one Claritin 10mg tablet earlier in the day. I recommended taking some Benadryl and the patient agreed. I gave him 4 teaspoonsful of Benadryl Elixir (50mg). I ask the patient to remain in the waiting area for 30 minutes or so and afterwards to let me know how he was doing. He didn't return to the pharmacy counter and was gone before a could speak to him. I called his mobile number and left him a detailed message and to call me if he had any further concerns. I didn't hear back from the patient.

Swollen eye lids (top and bottom), rash on face, hands, necks. Itchy all over. Throat tenderness and tight feeling

EMT's looked after patient/checked vitals and everything was okay. Patient states he has a fear of needles. Called patient a few hours after the incident to check

Pt received first dose of Moderna covid vaccine and during observation period of 3 minutes, pt stated "I feel funny in my chest and a little shortness of breath." VS taken= P-102, BP- 149/98, RR- 20. Skin warm and dry. No n/v noted. No c/o AP or numbness to hands or legs. Mother with pt. Notified lead RN, LVN of situation and wheeled downstairs to facilities urgent care.

mainly noted flushing of skin mainly on both forearm- slightly itching/ sensitive sensation - no hives , no edema noted - no other s/s of any breathing or swelling in oral mucosal area - remained alert no other stress -- recommended Benadryl OTC and pt was fine with recommendation and will contact to see pcp if any other worsening issues -- mainly want to know how to manage his second vaccination - what advise to give - recommend him to take Benadryl prior to next vaccine , or is he safe to get his second vaccination?

Approx. 5 minutes following vaccine administration the patient developed a transient loss of consciousness resulting in a fall causing a small abrasion to his left forehead. The patient regained consciousness immediately. He was alert and oriented. No witnessed convulsions were reported. He denied any further neurological symptoms. He reported that he did eat breakfast. He denied symptoms of anaphylaxis or severe allergy symptoms. By this time, paramedics arrived and the his blood pressure (and orthostatics), pulse, blood sugar, pulse oximetry, EKG findings were all within normal limits. He refused to be taken to the Emergency Room as well as treatment for the head abrasion. There were no physical signs of fracture. The patient ambulated out of the clinical without difficulty, accompanied by his girlfriend. S/sx of concussion were discussed w/ patient and he or his family members are instructed to call 911 should he development further syncope or other neurological symptoms.

Pain at injection site; fever 100.4-101.8 F overnight; fatigue; chills; sweat; cough (might be due to seasonal allergies)

After 2nd dose (May 3rd)...May 6th started to notice swelling in top half of both hands, stomach and chest developed hives and a rash. Not very bothersome, hands are very warm and extremely inflamed. Took allergy medications, they haven't done anything. No reactions after 1st dose, this is all after 2nd dose.

Around three days after vaccination, I (the patient) suddenly developed significant atopic dermatitis on my arms, legs, and genitals. The location of most severe symptoms was on the knee pits and elbow pits. The skin became red, bumpy, broken, and leaked a significant amount of yellow/clear fluid. The symptoms were not unlike poison ivy but less localized. I (the patient) had not experienced similar symptoms at any point prior and was in good health/not immunocompromised at the time of vaccination. Similar symptoms were not experienced at the time of the first dose being administered. Benadryl and a hydrocortisone cream were used to alleviate symptoms with little success. The symptoms have continued for around a week and have showed signs of improvement.

lightheadedness/ tingling sensation in head, fatigue, loose stools, sore throat, nasal congestion, dry itchy skin. all started about a week after vaccination. patient evaluated one week after symptom onset. no concerning findings

Arm soreness/aches started about 6hrs in (though injection sight was tender/sensitive to the touch pain became mostly localized around the armpit and shoulder area. Where the pectoral connects under the arm), then around 8hrs a headache and chills started. Chills and headache progressed and got significantly worse throughout the night. By around18hrs headache had progressed to a migraine and nausea started (likely due to the migraine). Simply sitting up in bed was enough to push nausea to vomiting at which the migraine subsided for about 30min-1hr. Chills and aching arm persisted. By 24hrs headache/migraine was nearly completely gone and chills had subsided. Arm pain persisted and got worse. Noticed around this time that there seemed to be mild swelling in the armpit. Around 48hrs in armpit area is still feels slightly swollen and causes a good bit a pain depending on how arm moves/rests. Injection sight still lightly tender. (currently the only symptom persisting)

Moderna COVID-19 Vaccine EUA Symptoms: Muscle tenderness at vaccination site, general achiness, fatigue, occasional difficulty breathing (felt like someone squeezing on my diaphragm and made it uncomfortable breathing for short periods of time. Started in the evening time, lasted a minute max each time, and occurred around 5 times before ceasing). Symptoms ended around 9 pm the same day they began

Patient have 2nd Moderna vaccine administered at 2:02 pm . At 2:30 pm patient reported an itchy skin, vague tingling and mild throat closing. Patient was accessed by P.Bergen PA and Benadryl 25mg was administered per his order. Patient was sent to ED via ambulance.

Acute diffuse urticaria starting one day after administration of Covid-19 vaccine, (Moderna), mRNA, LNP-s, intramuscular pf injection (0.5 mL dose) and persisting for ~3 weeks at time of evaluation

Patient developed an itchy rash on the neck starting 3-4 days after receiving the Moderna vaccine. Advised patient to speak to doctor about the rash.

Patient states that he received the first Moderna short and shortly after developed a significant rash on left arm and was prescribed Benadryl. Patient also had a spastic reaction were he unintentionally threw his plate up in the air and subsequently blacked out.

HPI:23-year-old male with chief complaint of rash. He reports it started on Saturday and has spread mildly. He has red itchy bumps to his hands and elbows and feet. Denies any new lotions or soaps or changes in detergents. He did have this happen once on his elbows years ago but it resolved on its own. He has been taking Benadryl for the itching but it helps for a short period of time.

Less than 24 hours after the second dose of the Moderna vaccine, I developed tingling and sensitivity all over my body, fatigue, and tachycardia. I reported to the ED of UPMC Shadyside with a bmp of 130. My heart rate remained above 100 bmp throughout the course of the day. The ED team did not find signs of infection, blood clots, or heart inflammation after a multitude of lab tests. After a series of IV fluids and pain medications they were unable to lower my heart rate or find a cause and so I was discharged with instructions to hydrate and rest. The following day my heart rate remained elevated from my typical resting rate. It is not until today (6/27/2021), nearly 72 hours after the injection, that my heart rate has stabilized down to a more typical resting bpm for me.

Solid red oval going 3cm laterally each way from injection site, and 5cm above and below injection site. Heat and minor swelling around injection site. Pain in armpit, almost unnoticeable while resting. upon raising armpit pain worsens. Swelling in the armpit as well. Headache. (NO fever).

Mild erythema with complaints of itching at injection site 7 days after injection. Physician treated with Triamcinolone Acetonide 0.025%, Cream, Topical APPLY TO AFFECTED AREAS TOPICALLY TWICE A DAY FOR 2 DAYS

Patient experienced al large red rash measuring four inches by 2 inches at its largest. Rash was very warm and had some swelling.

If patient was vaccinated at another facility, they need to contact that facility , it is an expectation that the facility that gave the vaccine complete the VAERS report. Patient states a painful rash developed in the eft axilla accompanied by a single hard nodule about the size of a quarter 24 hrs post vaccination. I saw the patient on day two. Noted multiple flat erythemic rash at the left axillary pocket there is no palpable nodule but site is tender to touch. There is also a single flat discrete erythemic plaque located along the mid axillary line, right next to the serratus muscle. Vital sign is stable, member is afebrile, ROM is not affected. Member advised to take Tylenol for treatment of pain

Post vaccination after approx. 15 min. of observation pt. noted blotchy cheeks. 1616 HR 82, BP 157/110, O2 SAT 99% RA - No complaint of SOB or difficulty breathing. No pruitis. No chest pain or pressure. - back with blotchy redness, no hives, no difficulty swallowing - no history of HTN, multiple environmental allergies - A&Ox3, HRR, S1S2, LS clear throughout 1619 BP 174/112 - feels flushed - Benadryl 25 mg PO administered 1622 BP 164/107 1629 BP 144/101 1635 BP 161/112, HR 74, O2 SAT 99% RA - feels better, less flushed, not itchy, no new symptoms - rash on back fainter 1640 BP 151/106, HR 80 - "I feel fine." - Discussed risks associated with elevated BP, denies additional questions and concerns. Works at Bangs as EMS and feels comfortable monitoring at home. - Reviewed repeating benadryl 25 mg PO once home and monitoring rash, reviewed S/S progressive reaction. Stated good understanding and repeated feeling well. Ambulated out of clinic without difficulty.

Severe Headache beginning 1 hour after receiving the shot. Painful nausea began 4 hours after the shot, at 5 hours began vomiting. I experienced painful nausea and severe vomiting from Sunday night at 9 pm until the last time I vomited on Wednesday night at approximately 11 pm. Nausea lasted until approximately 7 pm on Thursday. (Total of 4 days). Intermittent high temperature/fever, the highest recorded temp was 101 on the second day. Experienced severe exhaustion, spent 18-20 hours a day in bed, and was still exhausted every day until Friday. Also experienced intermittent chills and body aching until Friday. Some pain and soreness at the site of the shot as well as throughout arm but was not severe.

Itchy hives appearing first on palms of both hands on 2/1/21, spreading over subsequent days to tops of hands, arms, lips, neck, and scalp.

First dose: Fatigue, chills, muscle soreness, headache for three days after administration. Second dose: Fatigue, muscle soreness, headache for two days after administration. Itchy, red rash on right arm for 5 days after administration.

Left arm and hand swelling within 15 minutes after first vaccine. Later that evening his left left was swollen also on the same side that he received his vaccine. 2-3 days his injection site was sore.

Pain in arm spreading throughout first few hours, by 14 hours post vaccination hives began appearing with increased intensity and number. By 15 hours post vaccination a mild headache and low fever of 100.8 developed. Treatment included diphenhydramine to ease antihistamine response and aspirin to lower fever and reduce severity of headache.

I did not feel anything shortly after the injection, but later in the evening, started to develop a normal post-vaccine soreness. I thought little of it and went to bed. I woke up just after 3am with chills, fever, and worsened arm pain, to the point that it restricted my arm's range of motion and speed of motion. Interestingly, the arm is not red or swollen, but is painful near the injection site, and the area around my clavicle on that side is somewhat swollen, but not red or painful (likely a lymph node). I was unable to get back to sleep due to these side effects (primarily chills and fever). After a few hours of trying to get back to sleep, I took some Tylenol and tried to eat breakfast. I was unable to keep much down, and had to arrange a day off of my internship, and was finally able to sleep on and off for the next few hours. The symptoms have somewhat lessened, but have not resolved, after about 22.5 hours post-injection. I fully expect this to resolve or mostly resolve by the 48 hour mark. I mostly found this unusual due to the severity of the reaction and after only the first dose, and wondered if my young age could have something to do with the clearly robust immune response so soon after inoculation.

Following the vaccine the patient began experiencing flu-like symptoms including headache and fatigue the same day. They subsided the next day, however the injection site began to swell, turning warm and red. A large portion of patients deltoid around administration site continues to be swollen and red 3 days following administration. The patient hasn't noticed the area getting any larger and is going to continue to monitor for any worsening symptoms. Patient was advised to seek medical care if symptoms worsened.

8 days post vaccine, I developed erythema ( about 10cmx5cm), pruritis, swelling, and mild tenderness over my injection site.

Small rash on my left bicep. This particular rash occurs occasionally for me, but I'm not sure what causes it.

7-8 days after shot, arm had a massive red spot around injection site. Area feels hot to the touch, painful to move / touch and it itches. Area is swollen as well.

Patient received the Moderna vaccine on 3.18.21. Patient did not report any issue until 3.27.21. Patient states that on 3.26.21, an area of "irritation" developed at the sight of injection, a small circular area that is pink/red, Patient denied any swelling, purulence, signs/symptoms of infection, warmth, or pain. Patient said it was only a little red and irritable. Patient took an antihistamine (Xyzal) this morning and mentioned the area has slightly improved. I informed patient I would follow up with him daily until resolution. Patient denied requiring immediate medical attention; however, I instructed patient to seek medical attention upon development of systemic signs of infection such as fever, chills, or spreading of redness around injection site.

after 5-7 minutes after the administration of the vaccine, patient approached the counter to report that he had fainted between the isles. in doing so, he fainted for and fell down. regained consciousness immediately. but patient was very warm and clammy to the touch.. patient complained that his skin felt burning and his right knee was hurting. Patient was tended until 911 personnel arrived.

Severe hives developed 10 days after injection. Treated with 4-day course of prescription antihistamines.

Pain in left arm. Locations vary but mostly located in bicep muscle and elbow. Sometimes spreads to upper arm and thumb. It has been happening for hours for almost a week now got worse on Saturday 27th and Sunday 28th respectively.

Waking up the day after my vaccination, my right arm was so sore I could barely lift it over my head. I also had minor chills and fever but that cleared up after I took acetaminophen. By Friday (two days after my vaccination) the soreness had reduced but I had developed a 4" x 3" rash around the injection site. It was both itchy and slightly painful, but not enough that I felt the need to take a painkiller. Today (five days after my vaccination) it has finally receded away.

Head to toe rash 4 hours after receiving vaccination. Also complained of sore throat and fever which in combination with rash lead patient to be evaluated in ED on 3/28

Pain in vaccine site arm evening of and day after getting vaccine; headache ~18+ hours after getting vaccine; fatigued day after getting vaccine

Severe rash on toes, feet, legs, mouth, hands, and upper arms. Swelling in feet and toes. Injection site has become very hot to touch. Rashes began on 04/03/2021 on just the back of heels but then spread quickly to top of feet and toes within just a few hours. Used diphenhydramine gel and cream on feet on the night of 04/03 to be able to sleep. Rash on injection site and on the same region of opposite arm intensified on 04/04/2021. Intense itchiness called for more ointment on both upper arms injection site still hot to touch and ring rash has become more visible. Foot and toe rash returned at 8 pm 04/04/2021 and is unrelenting. Bumps appearing on toes.

Patient got dose #2 of vaccine on 02APR21 at 1552. At 1740, patient complained of headache, dizziness, nausea and warm to touch. He laid down on a cot, had snacks and water. B/P 136/69, HR 70, O2 sat. 97%. He was observed by an RN for an additional 30 minutes before sitting up for 10-15 minutes at which time he stated he felt better. At 2000, he returned to the vaccine room complaining of nausea and extreme fatigue. He was given Benadryl 25 mg po. O2 sat. 98%, HR 94. Patient sent home.

Moderna COVID-19 Vaccine EUA Fatigue and pain at injection site starting at 8:25 PM. Fever developed over the course of the night; temperature 101.4 degrees when measured in the morning. Fever broke at around 2:00 PM. Moderate-to-severe headache, moderate joint pain, and fatigue lasting from waking at 7:15 AM until 4:30 PM. Some nausea.

Developed a skin rash that was localized to the back side of my arms. Since then the rash has worsened severely and spread to my neck, torso, and back. I have seemed medical care multiple times. Been given 2 different steroid creams, 3 different antibiotics. The most recent visit (2 days ago) I was given Pepcid and Zyrtec. At this point I cannot sleep due to the heat and pain coming from the sites of the rash and need something to resolve it

Pain in right arm around injection site Muscle pain and aches in joints overnight and throughout the next day

Severe vomiting (at least 8 times) and diarrhea in the evening. Elevated temperature (99.2-99.5°F) with chills and other fever-like symptoms. Muscle ache around injection site, but no redness or swelling. The morning after, experienced mild fever (temperature 100.3°F) and fatigue.

A 24 year old male who has completed a 15-minute observation period with the following signs and symptoms of an adverse reaction: loss of consiousness and other - dizziness. Pt endorsed he is very anxious about the vaccination. Had a hx of fainting spells during vaccinations. Pt endorsed that while he was on the chair, he felt anxious and dizzy, then tried to take deep breath and passed out. Pt was found on the ground for 5 seconds but regained consciousness right away. No headache, chest pain, SOB, swelling of lips/tongue/throat. Denies any urinary/bowel incontinence. PMHx: none Pt was sent to examination via wheelchair. Vital signs check. Given water for hydration and apple juice. Felt better. vital signs stable Have abrasion on right knee and left dorsal wrist. Physical exam unremarkable. No hematoma or laceration on the head Observed for additional 30 minutes More likely syncopal episode vasovagal This is not an allergic reaction to the vaccine

Arm pain at injection site, getting worse by the hour. Have not taken any pain relievers in order to provide unbiased data.

After patient got vaccinated. He was sitting down then stood up and felt faint, fell and hit his head on the concrete wall. He stated he tried to use his hand for balanced but missed and fell. Paramedics responded. Pt has laceration on right side of his forehead. Briefly out for a few seconds. Vitals are stable. Blood sugar normal. Paramedics stated he will need stitches. Paramedics advised him to go to hospital to get stitches but patient said he is okay. Pt stated his girlfriend will take him home.

Hives noted on face minutes after injection. Patient refused ER visit. Benadryl 25 mg po given per order for allergic reaction. Patient had good results and left 30 minutes later after hives resolved

Fever of 102.5 for over 6 hours beginning 4/15/21, rash/hives appearing after 1 hour on left arm (not arm that received the vaccination), stomach, legs, hives still present as of 3:30pm 4/16/21 on arm, stomach, legs. Sore throat, headache.

Skin: slightly raised circular maculopapular rash to palms, elbows, knees. Slightly tender to palpation. No induration

Two weeks after vaccination he developed pruritic itchy papules on the elbows and sore pink macules on the lateral digits.

Patient after the vaccination during the waiting period started having vision changes, dizziness, shortness of breath, itching, paleness, sweating, throat a little tight. 911 was called, and patient monitored very close with Epipen out and ready to use if needed. 911 team arrived to store patient had some water and was starting to feel better. Patient ended up feeling fine and it was thought after all tests to be an anxiety attack. No medication was given. Patient did not go to ER.

Swelling, redness and rash around the injection site appeared almost a week after the vaccine. Initial muscle pain and soreness started about 12 hours after the injection but disappeared after 3 days.

Health care worker reported that he had an adverse effect of the second dose of Moderna vaccine. He described normal side effects in the first two days following the second dose: fatigue, nausea, headache. Then noticed redness and a cluster of vesicles on the right upper forehead on 4/22/21. Diagnosed with Shingles and treated with antiviral medication.

Pain in arm at injection site started about 3 hours after injection. Full muscle pain and painful to lift or move arm for next 3 days. Dizziness/lightheaded starting around 7PM after moderate walk, then persisted for next 3 days. Fever of 99.5, strong chills and shaking, and mild nausea started at 10PM as I prepared for bed, persisted through the night, but chills and nausea gone in morning, and fever mildly subsided. Took NyQuil before going to bed to treat fever. Chills returned next night, but much milder than before and for shorter. Took ibuprofen, 2 tablets twice a day for next two days. Dizziness and mental fog persisted until 3 days after injection.

Rash on left arm starting 1.5 inches down from the injection site. Red spots ranging from small to large distributed on the arm towards the elbow. Red spots are warm to the touch and entire area is swollen. Injection site does not appear to be swollen at the first notice of the rash (2 days post second round vaccination).

At the injection site. Its been like this for 2-3 days. Very tender, felt some fatigue throughout the day but very mild. Redness.

Soreness and sharp pains in the muscle around the injection site a few hours after injection until 48 hours after injection. Acheness through out the entire body after 12 hours after injection until 48 hours after injection. Felt hot and feverish 12 hours after injection until 36 hours after injection

A bit of expected soreness around the injection site, and fatigue lasting up to 30 hours after the injection

patient describes facial swelling for several days after vaccine administration, a diffuse rash that has mostly resolved on arms and trunk of body , and persistent joint pain to the shoulders, hips, and knees

quarter sized red, swollen spot at injection site started 5 days post-admin; by 10 days spot was "baseball sized" per pt. only painful to touch. pt treating with tylenol, claritin, and benadryl

24-hour fever, rapidly increasing to 103, before 1 gram acetaminophen reduced fever to 100.5. Two (2) additional 1-gram doses of acetaminophen were taken. Fever onset at about 4:00am 4/27, and broke about 4:00am 4/28. Additional symptoms include headache, chills, fatigue, soreness at injection site. Almost resulted in clinic visit

Patient's mom called at pharmacy on 5/17/21 to report patient experienced side effects like muscle aches, headache, 101.8 fever at night on 5/15/21, spine and teeth ache and injection site was elevated and hot to touch. Mom said patient felt better after taking Tylenol and most side effects have subsided, but she just wanted to report about side effects. Patient feels much better today (5/17/21) so mom said they are not planning to make doctor visit.

While patient was in observation, he started complaining of itching at the injection site. He was offered diphenhydramine but refused. Observation was extended for an additional 20 minutes and symptom resolved. Patient was discharged home without complaints of any other symptoms.

The afternoon following the vaccine (5/7/21), he began to have a hard time breathing, at work, switching breathing between mouth and nose and felt warm. He is a dishwasher. By 5:00 pm, both of his arm muscles and stomach was cramping. He was having bigger problems breathing, and becoming woozy. The paramedics were called to his work, and he was transported to the ER as they felt he was having an allergic reaction. He had an IV, Tylenol for a fever, Atavan to relax, Benadryl for antihistamine as they could see his arms were red. The ER did urine and blood tests as well as a Covid test. He was also given oxygen to bring his oxygen levels back up

Rash on arms and legs. Super itchy at first and got milder after two days. Still persists after a week.

on 5/20/21 I noticed 3 yellow spots on my chest that look like bruises but didn't hurt to touch. Now on 5/24/21 I have another yellow spot right under my right eye and the 3 on my chest are still there. I dont technically now if it is the vaccine that caused this but it did happen right after i took it.

Injection site soreness, early the next morning had intense flu-like symptoms. Can't stay still, legs cramping, muscle spasms, hard breathing, stuffed nose, chills/extreme heat with body

Diagnosis: acute pericarditis and acute myocarditis. Patient received Moderna Covid19 vaccine 1st dose 4/21/2021 LOT 037A21B to left arm. Stated he had sore arm. Patient received Moderna Covid19 vaccine 2nd dose on Tuesday, 5/25/2021 LOT 044B21A to right arm. Tuesday afternoon he developed a fever but did not take his temperature. On Thursday morning (May 27) he was at rest and stated that's when he first felt chest pressure. Chest pain got worse since yesterday, prompting a visit to urgent care then urgent care sent patient to ED. Denies shortness of breath, dizziness, cough, or other associated symptoms. complains of anterior chest pain, does not radiate. non smoker. trop 3.79, CRP 35.4; sed rate 17, EKG . ED physician consulted with cardiologist. Patient given ibuprofen 600 mg PO x1 in ED and discharged on ibuprofen and will be following up with cardiologist. discharge dx by ED doc: acute pericarditis and acute myocarditis

Very painful rash believed to be herpes or shingles by various doctors. The bumpy painful rash appeared on my cheek in a perfect triangle that resembled a shingles rash. I also had very itchy painful bumps on my forehead, chin, ears, neck, in between fingers, and all over my privates. Was tested for both shingles and herpes and both came back negative. Was put on steroids by doctor and rash has gotten better but has yet to disappear. I am still sick from when I got the shot and am dealing with a feeling that feels like my whole body is bruised and painful to the touch. Rash and bumps are unknown by doctors and the one on my fingers appears to be spreading on my fingers.

Pt reports that 15 days after vaccination, he developed a erythematous, pruritic, hive-like rash over bilateral legs and left arm that cleared up after 24 hours without intervention. The following day (16 days post-vaccianation) he noticed a petechial rash over bilateral anterior thighs with no known injury to thighs and small amount of bleeding when brushing teeth.

Bilateral discoloration of the thumbs. Pt denies any symptoms associated with discoloration. Pt states discoloration has been the same, has not gone down or increased in size.

Moderna COVID-19 Vaccine Injection site pain and hardness. 100.4 degree Fahrenheit fever, chills, muscle pain. headache. body fatigue. By now, most symptoms have subsided. But, I still have injection site pain.

I felt really weak after my second dose. I was very sore and tender to even move. I was also stated to look puffy in my neck and chest area. I was diagnosed with Myositis and Rhabdomyolysis. I was hospitalized overnight to keep fluids flushing through me due to the muscle count found in my blood.

patient experience pain at site of reaction but also experience numbness at lower left jaw/upper left neck which started a little after an hour after administration. After patient got a good night's sleep, all symptoms disappeared after about 24 hours.

Headache from 6/20/21-6/24/21, Fever of 99.5-100.2, nausea, vomiting, chills, aches, arm still swollen around injection site. Symptoms were gone by 6/25/21

PATIENT DEVELOPED RASH ON ARM. PATIENT SAW DR AND WAS GIVEN A STEROID SHOT AND PRESCRIBED A MEDROL DOSEPAK TO COMPLETE AT HOME THAT WAS BROUGHT TO THE PHARMACY AND FILLED TODAY 7/9/21

Twelve days after COVID vaccination patient broke out in a rash over most of body. Called Public Health nurse who referred him to his PMD. Patient told would be reported to CDC.

Injection site tenderness: Started at 2 hours post vaccine, lasted approximately 48 hours Malaise: started at 4 hours post vaccine, lasted approximately 20hours Low-grade fever: started at 12 hours post vaccination, lasted approximately 12 hours Treated successfully with acetaminophen

Fever of 100.7F, body aches specifically upper and mid back pain, soreness at site of injection, chills, general discomfort, difficulty falling asleep

1 week and 2 days post vaccination I woke up with a tender reactive axillary lymph node in the left extremity. Around this same time erythema and a mild pruritus was noticed at vaccination site. Area was warm to the touch. No treatment was started.

01/02/21 itching all over body -hives developed except for neck and face; Benadryl dose, resolves then returns 6 hrs later. Hives have not resolved, hives developed on neck and face 01/03/21 in PM. Benadryl treatment continues

Slightly larger than golf ball sized swelling and redness at injection site. Swelling and redness only at injection site, no pain or reactions anywhere else.

Sore arm/injection site began roughly 4 hours after receiving vaccine. No treatment used. Pain continue into second day.

Hot flashes, chills, cold like symptoms, some elevated heart rate, injection site warm to the touch, red ring around injection sight the size of two quarters length wise, was feeling foggy headed.

Approximately 8 hours post injection, pt had fever, severe L deltoid pain, malaise, headache. Lasted for 12 hours. Pain decreases each day. Injection site pain still present somewhat day 3.

Injection site pain starting evening of 1/8/20. Mild swelling and severe pain at injection site morning of 1/9/20. Moderate progressing to severe headache, and intermittent chills all day; no fever. Reduced appetite. Mild muscle pain/stiffness.

After receipt of the vaccine, I developed a low grade fever the next day (1/7/21) that persisted throughout the day until it progressed into a fever of at least 101.3 F at night. It might have been higher, but that was the best reading I could get. It felt like the fever came on extremely fast bc all of a sudden I started shaking vigorously with rigors and chills. I couldn't get out of bed without experiencing the most uncomfortable chills I've ever experienced. I previously had Covid in early November, and this fever felt worse than what I experienced when I actually had Covid. Also, I experienced moderate to severe pain in my left arm on 1/7/21 at the injection site, which was far worse than any other muscle soreness I've experienced from any other vaccine I've received. The hour preceding the sudden fever attack, my left arm was feeling blazing hot, looked a little red, and swollen. I also experienced a strange allodynia type pain, whereby parts of my body hurt from simply touching them. Although I did not develop a significant rash, I experienced pruritus all over my body, predominantly affecting the areas around my ankles and wrists. I have a hx of multiple dermal filler injections in my lips. I don't think I had any problems with my lips after the injection. But other symptoms that I developed included a tickle in my throat and some shortness of air. On 1/8/21, I woke up drenched in sweat so I presumed that I broke my fever while sleeping. I have felt much better ever since. I'm a nurse practitioner student, and I had to miss a day of clinical on 1/8/21 bc of this reaction to the vaccine. I used ibuprofen the morning on 1/7/21 and Tylenol at night on 1/7/21 to bring down my fever

Red rash that started a couple inches below injection site appeared day 6 after vaccination. Rash grew fast for 24 hours until half of arm was covered, including injection site. Stopped growing after taking 5mg of zyrtec. 3 days post start of rash it is still there but getting lighter.

Fatigue resolved by rest lasted about 24 hours Tenderness at site of injection resolved in about 48 hours

Significant soreness around injection site began at noted time with body aches as well. A fever began at 8pm and peaked at 3am with a temperature of 102.8. This temperature was maintained until 7am when the fever began to subside.

Pruritis on left lateral upper arm around area of vaccination beginning 1/11/21. 14.5 x 7 cm erythematous, targetoid rash on left lateral upper arm at injection site and extending down arm, first noticed 1/14/21.

The location of the injection is very sore. I'm also experiencing mild shortness of breath and fatigue.

I received the vaccine on 1/15/21. I had soreness in my left arm after receiving the vaccine and it continued for a few days. I also had redness and swelling near the injection site, but it was very small. On 1/22/21, the redness and swelling became more moderate than mild and was very itchy. It also had bumps inside the rash margins. After it started to expand more on 1/23/21, I decided to go to urgent care the next day, Sunday 1/24/21. The NP reported that it was an allergic reaction, but it was very localized and didn't include any other organs. I was prescribed Prednisone and was to take it for the next five days. I will be starting the medication today.

A week After received the vaccine ,on Saturday 1/23/2021 I notice swelling and redness on the injection site and it was itchy. I did not take anything to treat, the next day I went to the emergency room and the gave me ibuprofen and claritin. but other then the swelling and the redness I seem to be fine no fever no other significant symptoms

Received vaccine around 8am. At 10am noticed very minor aches. By noon noticed aches progressing but still minor. By 4pm noticed aches and fatigue. By 8pm had body aches, malaise, headache, and pain at injection site. Middle of the night had fever of 101F and body aches/muscle pain, chills, headache. Fever was gone by morning (99F at 9:40am on 01/29/2021). Body aches are slowly going away.

The day following my first COVID vaccination, I had pain in my upper arm for 36 hours and could not lift it above my shoulder. I also experienced 6 hours of constant chills throughout my body, which came on suddenly around 1PM (24 hours after my vaccination) and was sent home early from work. During the 6 hours at home I believe I experienced body chills, I experienced what I believe to be brain fog. My coworkers described my motor skills (walking down a hallway) as slow and lethargic, and my thoughts being delayed. I do not consciously remember the 6 hour timeframe when I experienced these symptoms. Around 7PM that evening, I remember having a "coming to" moment where I remember the chills subsiding, and being able to think again. The following day I felt back to normal other than pain at the injection site.

About 4 hours after dose, started to have a flat, non-itchy rash develop on chest and upper abdomen. Has slowly gone away since, but there is still some of it left. About 8 hours after dose, had severe chills, likely fever (did not measure), and lots of shivering. Skin felt very sensitive all over body. Arm was already sore at this point. Fatigue, drowsiness. Felt like a flu. The next day, large red splotch on left arm at injection site, with red line extending into the armpit area. Swollen lymph nodes in neck. Previous symptoms resolved, except for chest rash. Two days later, large red splotch on left arm at injection site is now very itchy and warm, firm, and slightly raised.

Fever, chills, headache, muscle aches throughout body, sore at vaccination site with a short lived rash, fatigue. It was bad enough to halt daily activities (I canceled a phd interview because of how bad I felt). This occurred from late 2/4 through morning of 2/6. Taking tylenol seemed to help and I felt immensely better the two or three times my fever broke.

A couple hours after the shot, I began to experience some local area soreness. After seven hours, I began experiencing body aches, and before I went to bed around 9PM I was experiencing chills. Throughout the night I had a fever. When I woke up at 9:30AM the next day, I had a headache and slight body aches. I took some Tylenol and felt better at about 12PM that same day.

Developed fever, chills, headache, nausea, fatigue, and severe pain at the injection sight. The fever developed about 8 hours after taking the vaccine. I had previously had COVID back in August and this is what it felt like for me.

Began scratching itchy arms and torso around midnight. Itchiness gradually became more unbearable and widespread to whole body over an hour, and face became warm and filled with rashes. Went to the bathroom and noticed hives and redness all over body. Took Benedryl and took a warm shower and symptoms wore off after approx. 1 hour.

Member reports going home the night of the vaccine and having a fever of 103, with chills, and fatigue. The next day the individual noticed swelling in the face, hands were itchy and tingly, and also skin welts in the back and shoulder area. Member went to urgent care and was advised to buy Zyrtec which he stated helped. Member has since recovered and is doing fine.

Starting today on the left arm in the area of the vaccine, it is red, swollen, feels hot and there are small bumps on his shoulder where he received it. It is localized to that area.

COVID-19 Vaccine EUA. Painful/sore at site of injection; left arm approx. 2-3 hours after injection. Also within 2-3 lower back pain/soreness. Night of vaccine, shortness of breath, full body aches/soreness, fever, fatigue, headache, chills, and nausea which followed into next day (today).

Injected with 1st Dose of Moderna Covid-19 vaccine. Injection site was above deltoid muscle and next to shoulder bone. Felt pain almost immediately that same day. Continued to be in pain and was admitted to hospital where they conducted blood tests, X Ray and MRI. Determined it was SIRVA.

Complaining of left arm rash that went down to elbow area. Looked blotchy and warm to touch, denies pain.

After getting 2nd dose of Moderna Vaccine on 3/18/21 @ 1335, patient developed a blister next to the injection site on the left arm on the area covered by the band aid which was discovered by the patient at 1215 pm on 3/19/21. The blister has a clear fluid and is not causing much discomfort.

Had a sore left arm where the injection site was, after about an hour later i began to have a mild headache and became tired (i took 2 aleve liquid gel pain killers and that didn't do much). The next day the pain in my arm got about 50% better, and the head ache was gone right when I woke up. I still felt tired throughout the 2nd day and my left under eye lid began to start twitching for about a minute long every 30 minutes. On the 3rd day, I was no longer tired and my arm no longer hurt but the under eye lid twitch is still occurring and at the same frequency.

0927 vaccine dose 0947 reported diffuse itching and "felling like I do when I eat nuts" 0950 BP 120/74, HR 100, SpO2 99% (CTAB, no SOB, no rash or oral swelling) 0952 reported he felt his "throat closing" the same as prior episodes of anaphylaxis so epi 0.3mg given by me via auto injector IM right vastus lateralis and called 911 0954 BP 124/71, HR 84, RR 18, 99% RA 0956 50mg IM Benadryl right deltoid 1001 EMS arrived and patient transferred to ED

Delayed rash/swelling at injection site. Reaction began 9 days after vaccine. Pain, redness, numbness, itching, muscle weakness, pain in neck (also left side), fatigue. The rash/swelling is about 4x4 inches

Approximately 10 minutes post vaccination, the patient expressed feeling "funny" and that his throat felt tight and he felt he was about to start sweating. His hands appeared red and were warm to touch, no other flushing noted. Blood pressure and pulse elevated. Benadryl 50mg administered PO. After 45 minutes of observation, vital signs returned to normal, patient noted resolution of tight throat feeling, feeling "funny" and redness of hands. D/C'd home with education regarding epipen use and emergency reasons to contact 911.

1st dose: sore arm, redness and swelling at injection site. 2nd dose: Fever for 36 hours, rose above 104 for several hours, diarrhea, sweating, pain at injection site, lighted headed, headache, difficulty breathing,

Patient had a rash develop after the soreness in his arm went away (he approximated 3 days) after vaccine on his left arm. It is red, round about 2 and 1/2 to 3 inches in size, occasionally itchy. It is still present 9 days after it showed up. Has no other symptoms. Patient is going to take some benadryl to see if it will help with the local reaction and itching. If it does not resolve he is going to schedule a visit with his PCP. On his way to a health care coach visit. I urged him to mention it to them and am also going to contact his PCP for him.

PATIENT HAD A SHINGLE LIKE RASH ON THE LEFT SIDE OF HIS CHEST BY NIPPLE AND ARMPIT AREA. ON THE 20TH IT WAS JUST RED, BY MARCH 31ST THERE WERE RED BUMPS AND SCARRING. HE ALSO HAS 7 CIRCULAR SHAPED DARK RASH MARKS ON HIS BACK (THESE DEVELOPED MORE RECENTLY, EXACT DATE UKNOWN). PATIENT STATED ON MARCH 20TH THAT HE HAD NO PAIN, THE RASH WAS ITCHY AND SENSITIVE ONLY.

Injection site swelling, redness, and discomfort, fatigue, muscle soreness, chills and feverish symptoms (no fever, temperature never over 98.6)

A few days ago, I began to feel pain in my left arm where I had the injection, and it has swelled up and turned red. So far I have not treated this side effect.

PATIENT STATES 2 DAYS AFTER RECEIVING THE VACCINE HE STARTED TO EXERPERIENCE UTICARIA ON BOTH ARMS AND BACK. HE SELF TREATED WITH DIPHENHYDRAMINE 25 MG EVERY 4 TO 6 HOURS AND REPORTS THIS HAS HELPED TO IMPROVE THE ITCHING.

After getting the first dose of the COVID Vaccine @10:30am on 4/7/21, I only experienced a sore arm for the rest of the day. Next morning around 10am, I woke up with hives on my left and right forearm/elbow area, and on my left and right knee area. No other symptoms besides the sore left arm. I took one anti-histamine pill (diphenhydramine hcl 25mg) every 6 hours, and applied cortizone (hydrocortizone) on the areas mentioned every 6 hours as well. It helps, but it comes back every 6 hours. I am worried if it will affect my status on getting the second dose of the COVID Vaccine. I also had almonds, but I never had a history on allergies related to nuts, dairy, and bread.

Swelling/knot at site of injection, mild itching. Appeared late in the day, nine days post-vaccination. No other symptoms currently.

Very sensitive skin starting at about 9:00 am until late in the evening. Clothing was annoying to have touching my legs especially.

Developed headache, tachycardia, painful heartbeat / strong feeling of heart beat in upper chest/throat, moderate nausea, sore injection site. Symptoms cleared after 48 hours, except for sore injection site, which cleared after 72 hours. Moderate nausea was treated with Zofran ODT during the first 48 hours of symptoms, prescription from urgent care.

I started experiencing mild soreness around the injection site on the arm the next day after the shot and it lasted for a total of 2 days( which is very normal) . Then almost after a week I noticed a resurgence of very mild (weaker than the first time) soreness which lasted for one day and then disappeared again. As of now, I do not feel any soreness of pain. I am reporting this only because I felt it might add to the existing corpus of knowledge we have about the side-effects of the Moderna vaccine.

9 days after receiving the vaccination the patient developed a rash on hands and feet. It has progressed to his arms, legs, chest, and stomach. The rash has welts and is itchy and even hurts. Patient is currently taking Benadryl and Zyrtec. He has not seen a doctor yet.

I was poked twice, the vaccine was administered on the second poke. After the first time he put the needle into my shoulder, he said that he "hit something hard" and didn't "want to just stab through it." He immediately poked me again and gave me the vaccine. Both pokes were much higher than usual. The morning after I had received the vaccine, my arm was in excruciating pain and my range of motion was almost nonexistent. I expected this side effect, but I did not expect it to last this long. It has been 3 days and my arm is still nearly immobile and testing the mobility is very painful. I haven't taken any medication since the adverse event. I visited an urgent care facility today and was told to fill out this form during the consultation.

Burning feeling in the left wrist. Throbbing from left wrist moving down to left hand. Lasted 30-45 minutes. Started within one hour after the vaccine administration. Sensations subsided afterwards.

Pain and bruising in injection site (immediate and continuous as of 10:00PM 4/21/2021). Chills (started around 7:00PM 4/21/2021). Fatigue (started around 7:00PM 4/21/2021).

Arm pain after the shot. Last and worsen over a day. Very painful now. ~24hr after the shot Flu like symptoms: muscle pain all over the body, headache, skin very sensitive/irritable. Whole body is weak. Mild lower abdominal pain. Affected normal life by a large degree.

Found 5 small red, blistered-like bumps on the right side of my abdomen. Was itchy and painful to scratch. Was under the impression it was a spider bite, but over time I am concerned it could've been shingles. I have had shingles before in the exact same location as these bumps, which makes me skeptical it was a spider bite. The bumps occurred approximately 4 days after my 1st shot. Bumps are nearly healed now and I did not seek medical treatment.

Patient is a 25 year old male A & O x4 with a chief complaint of a syncopal episode. Patient was sitting in a chair in the observation area approximately 10 minute post vaccine. Patient struck his head when he fell and presented with a small abrasion on the top of his head complaining of neck pain 1/10. Cervical spine immediately immobilized and EMS activated. Patient stated that he has had similar events in the past when he had his blood drawn. Patient denied any other medical history, no allergies and stated that he takes no medications. Vital signs obtained with all vitals within normal limits. Patient stated that he felt slightly dizzy but denied any other complaints. Negative chest pain, SOB, weakness, blurred vision, tinnitus, nausea and no sign of allergic reaction. Patient had equal grips and pushes with no signs of stroke. Medic Engine with EMT arrived and assumed patient care. Patient went via ambulance Hospital.

Vaccine (Moderna) given on 4/2 On 4/17, patient noted back pain radiating toward abdomen. Developed rash in same area of pain on 4/19- Seen on 4/20 and diagnosed with Herpes Zoster. Followed on 4/23 with same diagnosis.

The patient developed high fever to over 103F on the day of vaccine. 2 days later, he developed a red rash on the right side of his forehead, and in the next 4 days, swelling of right side of his face, right cheek, right temple, mandible, right ear, with difficulty of chewing, and some decrease in hearing in the right ear. The patient never had dermal fillers. I saw the patient today and put the patient on Prednisone and Keflex empirically, and referred him to dermatologist and plastic surgeon for 2nd opinion.

Redness/swelling of arm 24 hours after shot. Fatigue/tiredness next morning (12 hours after). Flu-like symptoms (nausea headache) and body aches next afternoon (24 hours after).

Noticeable swelling at injection site accompanied by increased surface temperature beginning one week after initial injection - "Covid arm"

Patient is a 25-year-old male with past medical history of depression with anxiety, esophagitis and hemorrhoids. Patient presented with petechial rashes all throughout his body started on Friday 4/9 after getting a tattoo the day prior and his first moderna covid vaccine 3/28. On admission patient was found to have a platelet count of 1. Heme/ onc was consulted and thrombocytopenia was deemed secondary to ITP. Hepatitis B panel, showed previous hepatitis B infection, although results may be inaccurate due to patient receiving IVIG prior to hepatitis panel being done. Patient also had an equivocal syphilis result, which also could be a false positive due to immune response. Patient received 2 IVIG, 4 days of Decadron, and 1 dose of Nplate. Patient did not receive rituximab, due to stable increasing platelet count. Patient received hepatitis-B prophylaxis for 2 days in case he were to receive rituximab. Platelet count now stable at 25, to follow-up with hematology in 1 week outpatient for repeat CBC. Patient also to follow-up with his Primary Care Physician in 1 week.

Patient felt like fainting about 10 mintues after dose. Sweating, ears ringing, . Patient fell as a result of dizziness and suffered a few lacerations to arm, bumped his head and complained of pain. 911 was called and patient was sent to Emergency room per ambulance for a checkup.

I've had both moderna doses properly spaced. No anaphylaxis at either time given. About 5 days after both doses I started breaking out with a itchy full body rash that looks like the measles. I'm currently on the second dose breakout it started 5 days after dose it has been 11 days and I still have a full body itchy rash. Im 25 I don't have healthcare and I can't afford to go to the doctor.

The day after the shot I had chills, fatigue, fever of 102.00. Monday the 10th my arm had a red spot, and a lump the size of a golf ball

Patient was sitting after receiving the vaccine. The patient had a hairy arm and the inject safe bandage had come up from the skin on one side. The injection site had some bleeding and the patient looked over and passed out at the sight of blood about 5 minutes after receiving the vaccine. The pharmacist went over and rolled the patient over and he regained consciousness immediately. After talking to the patient to make sure they were alert and ok they had patient slowly sit up and eventually sit in a chair. Paramedics also came and patient denied going with them to the ER. The patient remained sitting until they had his girl friend pick him up.

Large Red warm area around the vaccination spot appeared 6 days after receiving the vaccine. The spot is approx mentally 5.5" accross

The occurrence of additional non-serious events is detailed below. The patient's past medical history included Anxiety and Stomachache. Concomitant products included LAMOTRIGINE (LAMICTAL) for Anxiety, OMEPRAZOLE (PROTONIX [OMEPRAZOLE]) for Stomach discomfort. On 28-Mar-2021, the patient received first dose of mRNA-1273 (Moderna COVID-19 Vaccine) (unknown route) 1 dosage form. On 12-Apr-2021, the patient experienced RASH (Petechia rash) and FATIGUE (Tiredness). On an unknown date, the patient experienced THROMBOCYTOPENIA (thrombocytopenia) (seriousness criteria hospitalization and medically significant) and IMMUNE THROMBOCYTOPENIA (ITP) (seriousness criteria hospitalization and medically significant). The patient was treated with IMMUNOGLOBULIN HUMAN NORMAL ongoing since an unknown date at a dose of 1 dosage form. At the time of the report, THROMBOCYTOPENIA (thrombocytopenia), IMMUNE THROMBOCYTOPENIA (ITP), RASH (Petechia rash) and FATIGUE (Tiredness) outcome was unknown. The action taken with mRNA-1273 (Moderna COVID-19 Vaccine) (Unknown) was unknown. No laboratory details was given. Reportedly, the patient had to go to the hospital for a week and was still being monitored. Approximately 2 weeks post-first shot I developed an outbreak of hives on both hands (clear raised red circles, itchy). I have never had hives of any sort before. The hives lasted approximately 2 days, then disappeared.

Patient was walking in waiting room, passed out, fell backwards and hit his head on a chair. When I came to the waiting room patient was actively seizing and bleeding from a laceration on the posterior part of his scalp. Patient moved from under chair and placed in recovery position. Active seizure time from my arrival was 30 seconds. Patient was minimally responsive and postictal for approximately 5 minutes and diaphoretic. Vital signs taken and oxygen placed on patient. Color returned to normal, and VSS. BS was 108 on glucometer. EMS called and patient transported out of facility. Patient is a 35 yo M with no sig pmhx presenting after syncopal event after receiving his COVID vaccine. Fell from standing, denies dizziness, SOB, CP, or other symptoms prior to the event. He hit the back of his head when he fell. He presented to the ED, at which time he felt very nauseous and per notes he was confused and perseverating. Admitted to the SICU for R frontal IPH and R frontal SDH.

6/17 Rash on hands and sore throat 6/18-6/20 rash on hands, sore throat, headache, body aches, tiredness, brief high heart rate, slight cough

Chills, fever, hive like rash all over body a week after, and smells and tastes are no longer the same. There is a certain taste and smell (which are the same) that quite a few foods and other things taste and smell like.

Patient had received vaccination, He was standing in front of "drop off" to get his appointment for the second immunization and fainted. He went to the floor flat on his back. The tech called my name and I went out the door to find him on the floor in a semi-conscious state. When I knelt beside him, his eyes opened and I asked him several questions to establish that he knew who he was and where he was at. I asked him to remain on the floor until I could verify that he had no injuries to his head or limbs. Once that was established, I helped him stand and provided a chair for him. He stated that he didn't know why he fainted. I asked if he had eaten . He said he was fasting today. I sent the tech to get him some orange juice and a roll, which he consumed. most of. I kept him for another 20 minutes. he said he felt good and would eat a meal at home. I called him at his residence at 7:45pm this evening. He stated that he felt good except for the sore arm where I administered his vaccination.
