6 hours after vaccine developed fatigue and mild soreness. Over night had severe muscle and joint pain. The next day muscle/joint pain continued with headache, fatigue, fever, chills, and my urine turned dark brown. The next day fever went away and all symptoms improved except the urine continued to be brown. The following day I was seen at the ER and was diagnosed with hematuria of unknown cause. I have to have a CT scan on Monday to evaluate further.

35 weeks 4 days pregnant at time of vaccination. 60 hours after injection (on 1-23-21 at 10:30 pm) developed fever of 103, severe chills and tremors, low blood pressure (80/60), high resting heart rate (115 beats per minute). Treated with acetaminophen. Symptoms continued for a week, with ongoing fever spikes of 103 and dips down to about 100. Requested antibiotics from OB on 1-28-21 and began taking amoxicillin that evening. Symptoms began to lessen by 1-29-21 and disappeared after 1-30-21. OB tested for UTI on 1-28-21; returned positive result on 2-4-21.

Bladder spasms , urgency and increased frequency of urination. This lasted several days but was very intense the evening of the injection. The above is a chronic condition following cancer treatment (hysterectomy, pelvic radiation), however is controlled with medication. Medication had been taken appropriately in the days prior to and after the vaccine. This could be explained day two, as there was an increase in water intake due to severe thirst, however no significant increase in water intake the day of the vaccine. Between 11 pm and 3 am urinated 8 times (typically urinate 0-1x/evening). After 3 am, experienced insomnia the rest of the evening (awake and alert) Odd taste in mouth (occurred immediately with injection and lasted about 20 min) Dizziness (started within min of receiving vaccine and lasted 2 days) Poor temperature regulation (started within 1-2 hours following vaccine was severe hot/cold with severe sweating for 3 days. Currently continues 1.5 weeks following injection, however at a lesser extent. Currently , I over-heat with minimal exertion.) Extreme fatigue -slept for 1.5 days following vaccine. Notable fatigue persists currently, which is 1.5 weeks following vaccine. Fatigue with work-related tasks extremely easily and with working out (lifting weights, unable to jog due to fatigue)

My stomach was hurting while urinating and there was blood in my urine. At another instance, there was a small blood clot.

Swelling of right arm around injection site at approximately 7:30pm 3/19/2021. Uncontrolled vomiting for 15 minutes began at 12:00am on 3/20/2021. Rigors, profuse sweating, two episodes of urinary incontinence during the vomiting. Soap like/chemical taste in mouth shortly prior to vomiting. Full body myalgias, lethargy, fatigue, and mild headache.

Within 7-10 days of vaccine administration, developed severe urinary irritation, negative to UTI/active infection. Symptoms worsened to the point where patient was unable to walk without pain. Self-treated with OTC Azo (phenazopyridine) until able to make a doctor's appointment. Possible exacerbation of interstitial cystitis.

1st day: lightheadedness, nauseam with fatigue and mild SOB. 2nd day: In addition to the above symptoms, she developed headache with pallor, continuous vomiting until she had dizziness. She went to the Emergency Room and told she had infection and she was given an cephalexin. Stayed with nausea, low-grade fever, fatigue, chills and headache until today 04/17/2021.

- Itchy hands and feet (but no visible rash) for about an hour beginning about 9 hours post-injection. - Jumbling up words when speaking for about 30 minutes (about 9 hours post-injection). - Injection site redness and extreme itching which increased over 48 hours then faded over the next 72 hours. The redness and itchiness returned on day 7 post-injection and remained visible for four weeks before it faded away leaving an indentation that lasted about two more weeks. - Diarrhea, bouts of dizziness, nausea, and vaginal blood spotting on post-injection days 1-3. - Difficulty thinking clearly and concentrating on tasks on post-injection days 1-3. - Hives on neck on post-injection day 7. The hives lasted about an hour. Also minor hives and itchiness on right side of forehead accompanying the more prominent neck hives. - Sharp burning sensation on bicep skin on injection arm on post-injection day 7. The sensation lasted less than a minute. - Bouts of dizziness returned on post-injection day 7. - Metallic taste in mouth began around 11 pm on post-injection day 9 and lasted for about four weeks. COVID-19 test was negative. - Abnormal vaginal bleeding for five days beginning on post-injection day 13. - Swollen lymph node in neck on side of injection beginning on post-injection day 15 and lasting a few days. - Sudden ringing in left ear (tinnitus) that lasted about a minute (did not record the date but it was within two weeks of injection). - Sensation of throat numbness (similar to dental numbing) that lasted about an hour (did not record the date but it was either evening of vaccination or next day). - Fatigue- minor fatigue lasted for about a week post-injection. - Minor hives on stomach appeared the night of post-injection day 16 and were gone by the next morning. - Trouble sleeping for two weeks beginning on day 14 post-injection. - Frequent urination for one week beginning on day 14 post-injection. - Loss of appetite for two weeks beginning on day 14 post-injection. - Feeling jittery/shaky, even upon waking, for two weeks beginning on day 14 post-injection. - Anxiety for about three to four weeks beginning on about post-injection day 21. Also some symptoms of depression (crying and feelings of hopelessness) during this time.

March 26th, 2021 I received the Moderna vaccine. After the Moderna vaccine, my arm felt very sore. I was dizzy and tired. On March 29,2021 I got a heavy menstrual cycle. It was the heaviest on March 31-April 2. (March 14th, 2021 I put in a new nuva ring, I should not have gotten my period) On March 29th, 20201 I went to the chiropractor. My lower back was hurting and I felt awful. On April 8th I woke up to bloody blisters in my mouth. The right side kept bleeding and wouldn?t clot. On April 9th, I developed inflamed blood vessels all throughout my body from my head to my feet called purpura and petechiae. I called doctor immediately because bruises and red dots were spreading quickly. He sent to me immediately to get labs drawn at the hospital. My platelets were critically low and I was told to head to the ER. From there I was admitted. On April 9th, 2021, my blood platelets were 2,000. They are supposed to be 150,000-450,000 Prior to the vaccine, a full physical with labs were done by physician on March 22. My platelets were 210,000. Everything in my labs were perfect. Currently I am battling kidney stones. I had surgery on May 10th. I?ve never had a kidney stone in my life.

Hematuria lasting approximately 36-48 hours, with mild lower back pain and muscle fatigue. Emergency room care of IV fluids.

I was ok then about 6hours from getting second shot I started to get really sore and then around 930 pm I started to get a fever shaking n weak then around 1100pm I was really sick bad migraine headache and dizziness and high fever 102.7 n then I threw up 3 times within the night of 6/26-6/27 and I couldn?t hold my urine so I pee on myself while throwing up early morning I couldn?t sleep shaking and having to stretch n yawn a lot. I couldn?t keep anything down. Then 6-27-21 all day I had a fever couldn?t get out of bed except for bathroom and then my husband had to either carry me or guide me. I slept on n off all day but only for like 10-30 minutes like I had insomnia??? Now it?s 6/28/21 n I?m doing a very little better, bad headache migraine n weak my chest is tight n heart jumping around here n then. I just want to feel better this really hit me hard I haven?t been this sick since I was a little girl and had flu n pneumonia n bronchitis and was hospitalized for that I?m a strong woman but this vaccine really put me down n put me down hard. I have never had any other vaccine to make me sick like I am now. Anyhow I just wanted to report it.

The occurrence of additional non-serious events is detailed below. Concurrent medical conditions included Drug allergy, Drug allergy and Penicillin allergy. On 15-Apr-2021, received second dose of mRNA-1273 (Moderna COVID-19 Vaccine) (unknown route) dosage was changed to 1 dosage form. Received the first vaccine 1/10/21 and had no side affects. Received the second vaccine 2/7/21 @ 0830. At 1500, had a 101.0 fever, full body aches, chills, and all over body shaking. Concentrated, Dark, malodorous urine. Lasted on and off until 0700 2/9. Extreme Fatigue following on 2/9. At 2100 on 2/9, noticed bilateral hand swelling and discoloration (Purple and blue hands, with visible veins, ) BP taken on home BP cuff 159/95, HR 80 (no history of HTN).

Pain and burning with urination, started 28 hours after the shot. Ongoing for 24 hours. Resolved with lots of hydration.

Next day after second dose dark brown urine testing positive for blood now in emergency room being evaluated positive large blood urine awaiting bloodwork and ct scan

Swollen itchy rash around injection site 13 days after vaccine. Also, extremely low body temp (under 93°). Nausea, headache, kidney pain, felt loopy. All occurred at time the rash was discovered.

Few hours later my right arm is extremely painful with any movement. Woke up the next morning feeling very achy in my bones. Chills. Peeing very frequently (every 30-60 minutes). Feels like i have the flu (I don't, I've been vaccinated for that already this season)

Fever, pain all over the body, extreme fatigue and brain fog for the first 24 hrs, then fever and pain lessened a bit but remained until 4/9/21. Brain fog and fatigue for 4/10-11. Body aches, headache, top of left shoulder got a bump that really hurt (not the site of injection). Still painful two days later but as of now bump feels smaller. Pain in left kidney when trying to lift or bend at the end of the day. I drank lots of water and took Tylenol to help.

I started feeling discomfort in bladder area the night after I got the Moderna vaccine along with frequent urination. I called my doctor and went to get a urine culture done. I am still waiting for the doctor?s response. I was perfectly fine before I got the vaccine.

So there is something going on with my stomach, I woke up one morning and went to the restroom and my urine was really dark. I went to ER and they said I was severely dehydrated. When I went home I used the bathroom and it was burning and hurting really bad so I went to the DR again. They did some tests and the DR thinks it is an ulcer. Next I am looking for a gastroenterologist to see. They gave me some nausea medicine to help me be able to eat. My stomach is so sore and I am not really getting around and overall not feeling well.

10 mins after receiving the vaccine, my right hand started to swell a bit and had pain in the wrist and I got the injection in my left arm. Them I went to urinate and it was painful that continued until this morning. My body became itchy all over my blood pressure was 184/107. I was having heart palpitations. I got queasy and didn't feel well. I was given benadryl and zolfran. My blood pressure went down to 151/92 after a bit then after an hour I was still itchy, blood pressure was 133/ 84 but then my throat left side tried to hurt and I Felt like something was stuck in my throat. I waited another 30 thirty mins and left. My left side of throat hurts a little and my left hand still hurts and a little itchy

Almost passed out in bathroom, had to get help to get back to bed. Foot cramps. Had been to the bathrroom 3 times that night w/ pale clear urine. Had a small dinner the evening before. Drank Gatorade the following morning.

She was sent to emergency room Feb 2, 2021 by her PCP after calling him with headache, found elevated BP by school nurse, 163/95, vomited x3, left hand numbness but resolved upon arrival, decreased vision brief right visual disturbance. denies fever nasal congestion, sore throat, cough, chest pain. EKG NSR 95, head CT-no acute intracranial abnormality, UA and blood work completed

Episode required call to 911 and assistance of EMS to transport patient to the hospital emergency room for evaluation.

Severe kidney pain that night, so bad it woke me up and I couldn't sleep from it. It felt like I had been punched in my lower back for days. It's the worst kidney pain I ever felt.

Experience sharp and ongoing pain immediately after urinating. As my mother had UTI a couple of days after getting her Moderna shot, I suspected the same. But went to ER bc of how sudden pain occurred and because of closeness to me taking the second vaccine.

1st Moderna shot was 1/4/21. Had a syncope episode/potential seizure on 1/13/21 - unwitnessed loss of consciousness for a short period of time with loss of urinary continence. 2nd Moderna COVID shot on 1/29/21 with no reactions. My doctors evaluating me did not consider this as a COVID vaccine reaction, even though it did occur after the first vaccination. I am a nurse with a doctorate degree and do not believe the seizure was the reaction of the COVID vaccine. I was under extreme amounts of stress at work at the time. I also have a prior medical history of syncope episodes when I was a teenager (25+ years ago).

The next day after the vaccine I developed a severe migrane, nausea and vomiting for about 3 days. Then the Tues or Wed of the next week another UTI developed but resolved on its on.

A week after the vaccine I developed a UTI which was treated with antibiotics and resolved within 5 days after.

Lower part of arm was injected; my arm was sore about 12 hours after the vaccine. On Thursday, the 21st, I thought I was coming down with UTI with blood in my urine, and frequent urinating . I scheduled an appt for the 25th - but they wouldn't see me because I had COVID symptoms. Saturday, 23rd - started feeling unwell; next day, I felt like I got hit by a bus - achy, fever, cough out of nowhere - like a pneumonia cough and then my watch kept giving me alarms of my heart rate was too low. My oxygen was from 70-80 for about a week. I had an inhaler - and I used that. I used BOOST oxygen - and that helped my pulse oxygen level go up. My doctor, on Tuesday, the 2nd, (a virtual appt) put me on a couple of different antibiotics - for atypical pneumonia for one of them for COVID - Doxycycline; Decadron; and the other one was for Macrobid; Diflucan for UTI for two or three weeks I was recovering. I was sick for about four days more than my official isolation period - so off work for about 2 1/2 weeks. I went back to work on the 10th. Still to this day - I get a low heart rate alert. I delayed the second vaccine. Had the second vaccine on March 16, at 03:00PM.

Immediately after getting the shot- arm was sore and I was extremely exhausted. The day after getting the shot - I couldn?t lift my arm, had nausea, diarrhea that was golden in color, massive migraine, left kidney was in pain Day 3 - massive migraine, golden diarrhea, nausea, vomiting, left kidney pain

1030 am felt headache, pressure, extreme shaking chills, sore arm and oncoming body aches. By 2 pm added weak ess, dizziness, increase pain with aches, 6pm...extreme fatigue, nausea, chills, very extreme muscle weakness, allover bodyaches 8/10 pain with added mucsle spasms overall legs , abdominal, lung/chest, groin, to neck 9/10 sharp pains. 10 pm still continued ...unable to sleep due to pain and discomfort. 5am broke down and took 1 - 500 mg tylenol with continues use of heat/ice alternating previous 12 hours. After sleeping 5-830am.... reduction in aches and headache to 6/10... muscle spasms subsided. Continued fatigue and alot of sinus and ear pressure. 1- 500mg tyenol at 12 noon and napped 1220-230pm....woke up to strength back, mostly just sore arm where injected, increase in head ache and sinus pressure. 10 pm bed for night with 1- 500 mg tylenol and woke up every 2-3 hours during night with night sweats and sinus sneezing, runny nose, headache. 2nd day today post shot and slight headache, sore arm, some sinus pressure, slight fatigue. Last 2 days increase in urination triple usualy urination....constiaption.

Day 1 - joint pain in hands, some lymph pain in legs, lower back and kidney pain, fatigue Day 2 - lymph pain/swelling in injection site arm pit, fatigue, swelling has not subsided in 24 hours

Took the shot Wednesday April 7, 2021 at appx 530pm. Woke with stabbing intermittent pain on lower right quad of stomach Saturday April 10, 2021 at 745AM. It progressively got worse and I ran a low grade fever of 99.7 F. Drove to ER Sunday April 11, 2021. They (and I thought) appendicitis. Admitted asap. It was my kidneys. My ureter on the right caused the pain, and it spread to the right kidney in back. It is April 19, 2021 now. It eventually spread to my left ureter and left kidney. I had zero, ZERO illness and NO signs of a bladder infection prior. I've had 2 bladder infections in my 42 yrs and I went from fine to so sick I was scared. No trouble urinating, urine cloudy, pain etc. Just woke up literally 2 and a half days after my 1st shot and was sick. Am currently taking cefuroxime (Ceftin generic) 500mg every 12 hrs for 10 days I picked up Monday April 12. Also now took 12 mg Ivermectin (prescribed by a physician) Thursday April 15, 2021. A second dose Saturday April 17, 2021. Then I take it 2 weeks from the 17th, and 1 more time 2 weeks later.

It began with severe diarrhea and arm soreness, then came the extreme chills & fever (101F) and the worst body ache of my life. Then my arm and armpit and back and breast on the left side began to swell and swell and swell until I couldn't keep my my arm down because it hurts my armpit but I also could not lift is because of the pain my shoulder and armpit and breast are all very swollen and red and hot. The fever and swelling are now working it's way down my torso and hip and leg and foot all on the left side. On Wed 4/21 I went to Urgent Care and was tested for uti or kidney infection the result had blood in urine but as I am menstruating it was inconclusive. I had been give Ciprofloxacin to take Monday evening by a tele doc. Urgent Care ordered Prednisone 20mg 2 pills daily but the swelling keeps spreading. I have also been breaking out in sweats even just walking to the bathroom and am experiencing tiredness and severe loss of appetite and have lost 4 pound since 4/20 despite the swelling.

I felt the worst kidney pain I've ever experienced in my left kidney. Nausea and suddenly heavy limbs brought me to the bathroom floor. I experienced tremors. I struggled to breathe/hyperventilated. Both of my hands seized up, with the thumbs curling inward and fingers in a strange position. I could not straighten them out for several minutes. Then the pain traveled down to my legs. This entire episode lasted about 45 minutes. Afterward, I felt dehydrated and fatigued, even though I never actually vomited.

Severe vomiting, severe migraine, incontinence, inability to stand up without vomiting. Resolved after twelve hours.

Arm pain right after first dose, could not do much the first two days, Tylenol did not help at all. First dose triggered severe menstrual cramps two days later. Pain was an 8/9 on a scale of 1-10. Like labor pains. Trouble standing, walking, sitting comfortably. Full dose of Tylenol did not help. Heavy bleeding. Tongue and throat mild numbness at the height of pain. Extremely painful to pass stool and urinate. Worst pain lasted two days. 8 days later mild cramps persist, passing stool still uncomfortable. Day 7 cramps got worse for half a day. bleeding/spotting lasted 7 days (usually 3). Night 7 arm started to hurt again and itch, hives formed injection spot. Next morning (now) arm feels hot, itchy, sore and irritated. Still cramping in lower abdomen today.

Triggered migraines, couldn?t eat, Vomiting nausea diarrhea dehydration UTI kidney infection ended up in the hospital for five days

Pt experienced nausea , vomiting , diarrhea. Pt went to the hospital with kidney infection and UTI for 6 days. Pt has chronic migraine.

Pt reports probable seizure during sleep on night of 5/26/21-woke up in morning in "wet bed" due to urinary incontinence, one of the signs that she has had a seizure. States seizures usually only occur about once a year and can't remember when last seizure was but probably over a year ago. Also had right foot pain, possibly due to trying to get out of bed during night, no other sx.

Gross hematuria started approximately 31 hours after second vaccine. Developed fever, chills and body aches 12 hrs after vaccine, took Tylenol and Motrin to alleviate fever and body aches. Sought care in urgent care for gross hematuria. No treatment provided, was referred for CT scan and urology evaluation.

I woke up feeling just really crappy. My back and my sides were hurting. I was very sick to my stomach and did not get off the couch for a whole day. I went to the Dr and they found blood in my urine. I had a fever. I think it was 101. They prescribed me some medicines and I started feeling a little better. I started using my inhaler because I felt like I couldn't breathe. I had a lot of tightness in my chest. It felt like it was coming over me in a way. It felt like something was pressing on my sternum. When I would try to take a deep breath, everything hurt. I would get these waves of funk. My muscles would randomly contract in my leg. My entire leg was having spasms where I couldn't walk. Then the next day it happened with my other leg. It was like giant Charlie horses in both of my legs. I was thinking maybe I was magnesium and potassium deficient. I started taking vitamins for that. I was still not feeling great. I was sitting on the couch and my husband noticed that on the bottom of my foot there was a target lesion. It was white in the middle. It looked like a blister with a circle of black around it. I thought maybe I just hurt it on my own. Then he noticed more lesions all over my foot. It was all over both feet. He suggested I get it looked at so I went to the dermatologist. The dermatologist said that it was a target lesion and it was part of my immune response to the vaccine. He gave me some steroid cream and told me to apply it for 2 weeks. I then noticed on my arm that target lesions began forming where my surgery scar is. I also developed a UTI. I felt like my bladder was not emptying all the way and my back was hurting. They are starting to kind of go away, but they are still visible. I called Dr. and he asked me some questions. He wrote me a prescription for the next week. He said my immune response basically started at my feet and worked its way up. I am still having drainage and shortness of breath. I feel so sick I just want to lay on the couch. My mother had the same reaction. I also developed a rash on my chest.

So right after the second vaccine, I had problems, my kidneys were hurting and I had difficulty breathing. I had scheduled to get bloodwork done and after that work it showed that my kidneys were not functioning like they were supposed to. I told my DR that I felt like I had mono. My DR said tests results show that the results are similar to mono. I also lost like 8 pounds and had basically no strength. I was so tired and had to continue working, wearing two face masks and a shield because I work at a school. I was also dizzy and fatigued and would just sit down on the floor because I was getting so dizzy. I am also sleeping a lot. When school ended I was able to really just sleep a lot. Usually I would wake up on my own before 5AM but now I sleep in several hours past then. After medical tests the DR put me on two allergy medications and an antibiotic. The antibiotic was Azithromycin 500MG for five days 1x daily. The medications I am still having problems with my lungs/breathing and the tiredness. I also had a cough which seems to be getting better but I am still not recovered. I did not take the inhaler from the DR because I didn't want to take the antibiotic at the same time due to a worry about thrush.

Rapid heart rate and high blood pressure within exactly 24 hours of injection. Called 911 and then went to emergency room for help. Liquid ibuprofen was injected/given as treatment to slow/bring everything down. Did not go to work the next day due to being very tired from this event. Stayed home and rested. Followed up with my doctor within the next few days.

The symptoms started 12 hours after the shot, with chills, fatigue, severe muscle aches, headache, fever reaching 101 F, felt like was beaten up. Urine color was brown .Got a bit better after 30 hours, but temp didn?t go below 98.6 F. then After 48 hours notice rash on chest, 2 hours later the rash extended to neck, then to back and face. The fever got up to 101 again. Only to get better after injection with Benadryl and cortisone . 72 h later, rash better, but still there on chest with petechiae.

Symptoms the following day: 9/10 headache, all over body aches, nausea, dizziness, fatigue, urinary retention. The day after that I woke up with a skin reaction (red, blotchy) on my neck, chest, back, upper abdomen, and itching across my upper back.

Received vaccine on 3/3/21 at 2:30pm. Waited for 20 minutes at pharmacy for allergic reaction. Given the all clear, so headed home. Around 5pm, I started noticing how extremely sore my left arm was, almost as if somebody had hit it with a baseball bat (no bruising, redness, etc., just VERY sore). I also started noticing numbness and tingling in my left arm that traveled down the outside of my arm into my left hand and into my left pinky finger. As the evening progressed, I lost my appetite, developed a headache, felt extremely fatigued, was dizzy, had muscle and joint aches, had left kidney pain, and began to feel flushed and hot. I took my temperature and it showed I was running a low-grade fever of 98.7F (my normal temperature is anywhere between 96.9F and 97.1F). I went to bed around 9pm. Around 3am (which is now 3/4/21), I woke up with nausea, and my left arm/hand/finger really experiencing numbness and tingling (it's as if that portion of my arm/hand/finger was semi-paralyzed). I went to the bathroom and was on the verge of throwing up. I broke out into a cold, clammy sweat but did not vomit. I got back into bed and went back to sleep. Around 5am (still 3/4/21), I woke up again with nausea and my left arm/hand/finger still numb and tingling (semi-paralyzed). I went to the bathroom and was again on the verge of throwing up. I, again, broke out into a cold, clammy sweat but did not vomit. I got back into bed. I decided to take a Zofran around 6am for the nausea. I slept from 8am-12pm on 3/4/21 due to the Zofran. When I woke up, I called the pharmacist at pharmacy to notify them of my symptoms. The pharmacist everything I experienced was normal and should go away in 2-3 days (if they didn't then contact my regular doctor), that I could take Tylenol for any pain, and I still needed to keep my follow-up appointment for the second shot. The entire day on 3/4/21 from 12pm until I went to bed around 10pm, I still experienced an extremely sore arm, loss of appetite, slight headache, fatigue, dizziness, muscle and joint aches, left kidney pain, low-grade fever (for me), and the numbness/tingling/paralysis in my left hand and left pinky finger. I was able to sleep through the night, and woke up around 6am on 3/5/21. Currently, I am still experiencing an extremely sore arm, fatigue, and the numbness/tingling/paralysis in my left hand and left pinky finger. I am EXTREMELY concerned about the numbness/tingling/paralysis in my left hand and left pinky finger as it does not seem to be getting better.

Chills, body aches, headache, bones hurt all over, joint pain, severe back pain, kidney pain, fever, several fever blisters

Extreme pain in joints/body aches developed approximately 12 hours after dose with front/side headache similar to migraine pain level with nausea; treated with Nyquil Cough at 3:40 am, Tylenol-650 mg at 7:30 am, and both at 3:30 pm on 3/11. On 3/12, body pain/aches gone, but left with migraine type headache and fatigue. Treated with Tylenol-650 mg on 3/12 at 7:30 am. Fatigue continued until at least 10 pm 3/12 but gone on 3/13. Main reason for report: I had an ablation performed and tubal ligation in December 2018 with no break through bleeding at any time since. On Day 28 after first Moderna dose, there was noticeable cramping in low abdomen not experienced since ablation. On Day 32 from first dose-- the same day as the second Moderna dose, there was cramping again in low abdomen and menstrual fluid appearing in urine- more red in color. On Day 33 post-first dose and Day 1 post-second Moderna dose, there was more break-through menstrual fluid- very dark in color typical of prior "end of period" fluid with similar odor, with continued cramping. This continued, but lessoned on Day 2 post-second vaccine. I understand there can be failed ablations, but the timing seemed worth noting for you in case there is any pattern with impacts on menstruation.

2 days after the first vaccine I started having symptoms of a urinary tract infection. Called doctor and they prescribed me Nitrofurantoin for 5 days. I completely recovered. Now at almost 2 days after the second vaccine I'm showing some symptoms of a urinary tract infection - lower back pain; some pain in when urinating.

ANCA was reactivated by vaccine and is attacking my kidneys. I am getting a biopsy March 30. I go in for routine lab work every 8 weeks and these were the worst results I've ever received. High levels of Protein in urine and my GFR is down to 30

Day of tired; completely exhausted; Night of the shot awake every 2 hours had to use the bathroom (NOT USUAL) and either too hot or too cold. Following day fever of 101.0, body aches, tired, dizziness , and lightheaded.

First of all the shot itself hurt, injection hurt and my arm was VERY sore from the moment the needle hit me till about 48 hrs. later. About 6-7 hours after getting the shot I felt chills, muscle aches and headache. The next day I had a fever of 101.5, plus all the previously mentioned symptoms. Very dizzy all day and a little nauseas. All I could do was lay in bed, drink fluids and I took Tylenol mid day on 5/8/21. The Tylenol seemed to help me sleep and my fever came down to normal range, but then my kidneys and liver started hurting that night and the next day. Symptoms continued until late on 5/9/21. By 5/10/21 I feel basically normal.

On May 2nd started with symptoms of food poison, went to urgent care, gave her hydration and sent her home. On May 7th was taking to the hospital, look very weak and was confused, couldn't answer basic questions. They kept her in the hospital. On May 8th she had an apparent seizure and was was not until May 11 that she came out of it, being able to recognize people (though very weak). In the meantime her two kidneys stopped working. She continues to have seizures despite of the hospital giving her anti seizure medications. You could talk to the hospital more about the medical details.

Extreme weakness - had to go down the stairs on my bottom, resting every other step. Dizziness Frequent urination - every 20 minutes throughout the night Intense lower back pain throughout the night that stopped in the morning. Hypersensitivity Regular flu like symptoms - chills, aches, hot/cold, tired, no appetite Extreme symptoms were better by the morning. Flue like symptoms remained throughout the next day

At the very beginning I ended up with a bad cold as part of the symptoms. I was rushed to the ER with a Kidney and Bladder infection. The ER doctor prescribed antibiotics to treat the infections. I was in bed for three days because of the infections. I'm afraid to take the 2nd dosage because I do not want the same thing to happen.

Severe muscle aches, shaky, goosebumps all over despite layers of clothing, fever of 101, swelling from my shoulder down to my elbow along with red splotches that were scaly and itchy, my arm is hot to the touch and extremely painful, flank (kidney pain, left side)

Within an hour I had a headache. By 4 hours my arm was very sore. At 8-13 hours I felt like I had the flu with body aches, chills, and sweating. Between hours 14-44 my experience was intense with fever >102 (that was not responsive to Tylenol or Motrin), uncontrollable shaking with an episode of urinary incontinence, fainting, rash on chest/neck, right facial twitching, right arm twitching, nausea, vomiting, diarrhea, chills, profuse sweating, flashing lights in both eyes (open and closed), intense body aches, the sensation that my skin was burning, muscle spasms (back), joint pain (knees/hips), dizziness, agitation, and insomnia. After 44 hours the intense symptoms resolved and I had fatigue, right facial twitching, right arm pain/itching/redness. I went to see my doctor on 1/28/2021 to document my experience.

6 day long migraine, Weak, nausea, body chills, stiffness, dizzy, body aches all lasted 6 days. No fever though. After day 3, I went to hospital thinking I had Meningitis because my head hurt so bad and my neck was stiff. They ran an urinalysis and found a pretty bad UTI. The doctor said that the UTI exacerbated the side effects of shot. I don't know if that's true. I got I. V. Keflex and prescription. Migraine continued for another 3 or so days. I missed 4 days of work. Honestly the symptoms felt like I was doing chemo again. It was awful.

Really bad chills and boast aches with fever. 5 days later shingles with rash under right breast and severed back pain plus uti

Covid Arm on day 2-4, just below injection site. Urinary issues... similar to UTI (urgency at first then not wanting to go, just pain) but not the same pain feeling for 4+ days. Feverish and bad headache on day 5.

Acute interstitial pneumonitis J84.114 Patient did receive Benadryl 50 mg PO, Zofran 4 mg ODT, Depo-Medrol 80 mg IM, Solumedrol 62.5 mg IVP, NS 1L bolus IV, Ativan 2 mg IVP, and a Duoneb nebulizer treatment of symptoms while in the ED. 2. Pneumonitis, hypersensitivity J67.9 Patient accepted for ED to ED transfer by Dr. (ER attending) at 1555. Patient transferred by air to another ED.

I couldn't work and had arthritis in my hand and had flair ups in my joints and skin. In the chest, kidneys and liver. I had inflammation in my organs and throughout my entire body surface and internal. About a week in 7 or 8 days I started getting inflammation or irritation in the chest. It felt like my airways like it was triggering asthma. After 24 hours it went away. In week 2 my skin in my head and my scalp had an inflammation. The follicles were swollen, it was painful and hurt. After two days it went away and became normal. The real problem started in the middle near the end of week two. The hives started I was sitting on the chest and then my chest itched so bad. When I went in the bathroom and looked there were big red whelps on my chest. I took a Benadryl and after 3 hours it went down. Then my back had a big red area of hives on my back. The Hives covered my entire body and it was a huge problem. In addition I was taking 3 or 4 Benadryl a day. I sought medical care about it. I got into an immunologist/allergists and he did a bunch of labs and bloodwork. He was concerned and not sure if I should take the second shot. He did not give me a recommendation either way. Prescribed antiinflammation and skin cortisone cream.

My body hurt, I had fatigue, my tongue felt funny, I had numbness and tingling in my left arm and hands. Later on I started noticing headaches and blurry vision. I could barely hold my urine. I went to the doctor and they told me my sugar was high thats why I couldn't hold my urine and I never had these problems.

Had the Moderna vaccine on January 21,2021 @9:15am, the next day started having light tea colored urine in the morning and in the afternoon-amber colored urine. That night 11:00pm started urinating red colored urine which lasted until after 11:00pm of January 22,2021. On January 23,2021 in the morning-back to normal yellow urine. Reported this information to my nephrologist on January 23, 2021 @7:37am. Dr. responded via MyChart message: Ordered some lab work and recommend to report this information to CDC.

1/20/21 developed fever of 105 degrees, after Tylenol and Advil dropped to 104 which continued for 6 days. Also had nausea and vomiting which led to dehydration, hospitalized on 1/25/21 with Acute Cholecystitis without calculus, Hyperbilirubinemia, UTI, Acute kidney injury secondary to dehydration, Pyelonephritis. Surgery done for Cholecystectomy. 2 units of blood also administered.

Adverse reaction started with very dark urine ,fatigue. fever fluctuating and severe sweating ,shivers ,dizzy and back pain. I was very worried about the dark urine because I had history of hemolytic anemia. I went to ER and got diagnosed with Hemolytic Anemia after blood and urine test. Admitted to hospital overnight and dismissed with my request to do so with steroid prescription of steroid prednisone 20MG 3 tablets a day for 7days to suppress my immune response for for the vaccine that believed to be one which triggered the Hemolytic Anemia. following up with Hematologist on Monday with extra blood draw to monitor my situation. Still not feeling well with fatigue, back lower pain and dizzeness .i still look yellow and urine dark .

Had first vaccine 07Jan2021-within a couple of hours noticed an odd smell and color to my urine that lasted for about 48 hours. Had second vaccine 04Feb2021-the same thing occurred within a couple of hours and also lasted 48 hours

10 days after second dose awoke at 11:30 at night with nausea then after a few minutes began to itch and break out into hives then proceeded to get chest pressure and unable to breath until I passed out. Husband able to revive me 3 times until 911 came then went to urgent care Nausea with vomiting and chest pain, back pain, heart burn, and urinal incontinence. Sent to MC for NSTEMI work up of echo and cardiac stress test. Cardiologist said NSTEMI from low oxygen and hypotension.

Moderna vaccination #1: Low back around kidneys immediately tightened up. Got up 3-4 times in the night to urinate (which is not normal) FIRST THING IN THE MORNING WHEN GOT UP around 6:30/7:00am: body temperature felt extremely HOT and stopped being able to walk or function. Almost fell down from feeling dizzy/out of it. Had to drop to the ground and stay there for a while. Within about an hour felt mostly normal again. Did NOT take temperature or blood pressure, would have if I hadn't been in a state of fear/shock. Also itchiness in left deltoid 12 days after the vaccination, fingers on left hand accompanied this itchiness. Felt irritated in general at injection site up to two weeks after. Digestion affected - loose stools for the week after and increased gas up to a month after injection (current)

Three days after vaccination I noticed painful urination and frequent urination. Blood in urine on 01/15/2021. Telehealth consultation antibiotics prescribed Keflex 500mg/ 3X daily for 7 days. Prescription worked quickly. No history of bladder infection previously. Second dose only mild symptoms.

I had kidney stones that had to be removed not sure if connected to vaccine & happened right after shot. Happened suddenly right after I has symptoms

Sore at injection site for 3 days, then suddenly felt exhausted, weak, could barely stand because of leg weakness, dizzy, lightheaded, foggy thinking, urination problems and constipation. All lasted 5 days, I?m still feeling a bit dizzy but other symptoms have subsided for the most part.

pt presented in ER 3/17/21 with SOB and back pain; felt bad since vaccine given; UTI diagnosis-given keflex 500 tid and rocephin shot pt presented to clinic 3/19/21 with continuing back pain dx low back pain and candidia given methocarbamol, diflucan, and ibuprofen pt presented to clinic 3/26/21 with continuing back pain, difficulty urination. given ceftin and rocephin shot pt presented to ER 3/27/21 renal failure and expired within 10 min of arrival no fever documented at any visit

Patient received 2nd Moderna dose on 4/9/21 morning, on 4/10/21 morning, she noticed dark brown reddish urine, increased urinary urgency and frequency, and mild dysuria lasting for 3 days. Denies fever, chill, abdominal pain, nausea, vomiting. patient experienced similar discolored urine after the 1st Moderna dose as well, but lasted < 1 day, so she didn't seek care at that time. Patient was evaluated on 4/12/21, vital signs and physical assessment within normal limits. Patient was advised to have stat labs done, treated with cephalexin 500 mg BID x 7 days for UTI, advised to increase fluid intake, monitor symptoms and seek follow up care, repeat UA with culture and labs in 4 days. By 4/14/21, while on antibiotic, patient reports feeling better, urine turning yellow, urgency and frequency resolved.

10 mins post vaccine tachycardia. 8Hours later woke with myalgia, stiffness, nausea, bilateral pulsatile headache w/tinnitis. 20 Mins later in bath sudden onset of emesis, bladder & fecal incontinence, loss of vision seeing black spots,angina,dyspnea,followed by bronchorrhea. Transported to ED via ambulance and symptoms decreased. 2 hrs later stand up &feel faint, angina,dyspnea, urinary incontinence. facial paresthesia starting with tongue,then cheek, ears,whole left side of face, then left side of body. At same time severe pain in left calf and groin. Sensation of popping then cold wet at base of skull,increased pain on left side of neck. Symptoms decreased after Benadryl discharged from ED with diagnosis of vasovagal. Continued to have cardiac symptoms and neurological symptoms for 3 months.

Severe diarrhea, profuse sweating, migraine, insomnia, C-diff, UTI, chills, swollen glands in neck, fluid in ears and extreme upper back pain, HBP (not normally an issue), high heart rate

Later that evening, I began experiencing severe nerve pain and general unease. I have recurrent UTIs and experience a worsening of my existing nerve pain when I have UTIs - this may be "referred pain" which is common with spina bifida and paralysis. The pain worsened to the point that around 11 PM I got back out of bed and scheduled a telehealth visit to request antibiotics to treat the UTI, which were prescribed for me. My symptoms almost completely resolved by the next morning.

On 03/16 I started noticing some shaking. I wasn't too alarmed with it because it felt like chills. But, then on the evening of 03/17 I started noticing involuntary muscle movement. I was noticing involuntary stretching. It would wake me up from sleeping. As soon I started getting tired around 3pm in the afternoon it would start this odd stretching and it would usually resolve around the next morning. I experienced a lot of weakness and fatigue. I also had GI symptoms such as incomplete bowel movements and loose, watery stools. I had exercise intolerance. The night stretching increases as my physical movements increase during the day. I've heard heart palpitations. I had increased anxiety. I had night sweats. I had changes in taste. I've had tinitis. I've had ringing in my ears. I've had pins and needles and burning on my skin. I've had difficulties with thirst and urination. I was extremely thirsty. Seemed like a problem with those automatic responses. I've also had temperature intolerance. That increases my symptoms. I have lost about 12lbs which is like 10% of my body weight. I have gained it back thankfully. I have experienced excess salivation and some swallowing problems. Luckily that has not persisted. My face would become extremely flushed randomly. I have also had extreme anxiety. The flushing has seemed to resolve. I had insomnia with the night stretching and tremors. I saw a doctor for the first time on 03/26. I went to a neurology specialist. I saw him again on 05/13. I am scheduled to see him again in July. I also saw my PCP. I was referred to a sleep neurologist. My inflammatory response was more elevated. The specific inflammation test said CCL 5 (RANTES). That particular value was considered high and greater than 15,000. It indicated some inflammation. I went to the ER 04/16 and 04/19.

I experienced overactive bladder and excessive urination starting a couple weeks after the 2nd dose of Moderna vaccine for Covid-19. I then experienced fatigue and headache along with an early and excessive menstrual cycle lasting longer than normal by 10 days so far. The bleeding is on and off heavy and I am very concerned that my menstrual cycle is interrupted and still going on for 15 days.

A few hours post vaccine, developed splotches and a scratchy throat. Took a Benadryl which resolved those symptoms. Wed, woke up with terrible headache, nausea and kidney pain, no other urinary symptoms or fever. Took Tylenol and Advil alternating between the two. Thursday, had kidney pain and diffuse joint pain. Friday, joint pain improved, still had kidney pain and do as of today, 4 Jan 2021.

Within 4 hours of the vaccine developed fever and body aches. By day 4, was noted to have a ptosis and went to the ED. CT head negative as part of possible CVA work up. No other neurologic symptoms. Was also diagnosed with a UTI at the same time. Starting to feel better but ptosis continues. She will be seeing ophthalmology and neurology.

Fever, 102.7 fatigue, nausea, blood (probably myoglobin) in urine Treated with D5 and zofran in primary care office (treatment for any illness where oral intake cannot be maintained or if febrile and unable to maintain calories at home)

Finally urinated and it was dark in color; Induration of erythema about 3 inches; Acute renal failure; Felt so dry and realized I did not urinate; Disoriented; Sustained tachycardia for about 26 hours straight; Couldn't lay on left side; Induration of erythema about 3 inches; chills/rigors; Chills and sweats, back and forth; Severe tremor, rigors/Chills; Temperature was 104.1/ Fever persisted; Significant headache/Worst headache of her life; Body aches; Nausea; Vomiting; A spontaneous report was received from a consumer who is a physician's assistant) concerning 46 year old female patient. Medical history included cystic fibrosis. No relevant concomitant medication was reported. On an unspecified date, the patient received the first of their first planned doses of mRNA-1273 (lot number unknown) for prophylaxis of COVID-19 infection. On 04Feb2021, the patient received the second dose of mRNA-1273 (lot number 028L20A) intramuscularly for prophylaxis of COVID-19 infection. On 05Feb2021, the patient experienced severe tremor, rigors, temperature of 104.1, significant headache (described as worst headache in her life), persistent fever, body aches, nausea vomiting, chills and sweats back and forth. The patient was in bad all day 04Feb2021 (Thursday) through Saturday until 2pm. She felt dry and did not urinate, until after a bag of unspecified intravenous fluids, when she finally urinated in was dark in color (the patient considered herself to have acute renal failure). The patient also experienced sustained tachycardia for 26 hours, was disoriented, had induration of erythema about three inches at the vaccination site, and could not lay on her left side. The patient treated herself with ibuprofen and Tylenol for fever and body aches. The patient reported she was fine on 08Feb2021. Action taken with the mRNA-1273 in response to the events was not reported. The outcome of the events of acute kidney injury, urinary retention, tremor, pyrexia, headache, pain, nausea, vomiting, hyperhidrosis, chromaturia, tachycardia, disorientation, vaccination site induration, vaccination site erythema, hypokinesia, and chills was considered resolved.; Reporter's Comments: Based on the current available information and temporal association between the use of the product and the start date of the events , a causal relationship cannot be excluded.

Syncopical episode, hands felt numb, incontinent with urine, c/o couldnt move fingers. VS: BP 140/90, HR 65, O2 Sat 98%

Extremely sore arm. Fatigue and diarrhea and nausea. On the 13th, I started getting a UTI and had a Doctor's appointment for the UTI on the 18th of January. Put me on an antibiotic. The antibiotic didn't resolve the symptoms - I was having pains off and on for about 6 weeks. I went to my Dr - my OB, Feb 3 - did another Urinalysis - and that came out okay

Began with severe back pain on 3/04/21. I went to the ER and they checked heart because pain radiated to front. I went home and pain continued briefly the next day. By 3/6, I began to have numbness in my left leg, so again, I went to ER and they prescribed oral steroids and recommended MRI to be scheduled in back. My Monday 3/8 I was unable to walk and had no sensation from mid chest down. I could feel Pressure, but had no sense of temperature or sharpness. From that point I was admitted to the hospital. I spent a week on the 6th floor and a week on the inpatient rehab floor. They ruled out demyelinating disease that could cause the lesion that was found on my cervical spine from c6-T-2. The records indicate demyelinating disease vs post vaccine reaction. All tests for demyelinating diseases came back negative, so it is believed by the doctors that this was definitely caused by the Moderna vaccine. It caused a transverse myelitis reaction, which I am still dealing with. My walking is extremely slow, and the sensation issues persist, along with incontinence. I have all of my medical records and diagnosis for record. The hope is this will eventually correct, but for now, I am still in PT/OT twice a week.

Vasovagal Syncope starting five minutes after injection lasting about 20 minutes. BP fluctuated. Heart rate and O2 were normal. Monitored for approximately 30 minutes and then released. Abdominal pain started 3 hours after vaccination. Pain worsened and became severe over several hours, peaking with other flu-like symptoms and decreased over the next several days as other symptoms decreased. Pain was worse on left side about half way up rib cage and lasted for 5 days. Abdominal pain was worse and sharpened when twisting, rolling or bending torso. I can still feel a pulling sensation under ribs on left side off and on. Also starting about 15 hours after vaccination I had brownish colored urine that lasted about 36 hours. Saw FNP 10 days after vaccination. CBC, liver & kidney function blood work & urine dip stick tests all normal. They could not feel any enlargement on physical examination, but suspect spleen issue

Almost right after receiving the vaccine, I started to feel sore under my neck and my shoulder. The gave me an ice pack. I took 800mg of Motrin. The same night I started to feel like I had the flu. There was muscle and joint pain throughout my body. I had a headache and my stomach was hurting me. I developed a fever and it went as high as 105 degrees. I called the on-call doctor the next day and she said that I would probably be fine by Sunday. I did not feel better and my symptoms continued. My blood pressure became very high too, and I was advised to see my doctor, but it got better. My knees felt like they were on fire. My appetite went away come completely. On Sunday, I called the doctor again and told her how I was feeling. My doctor told me to go to the hospital to get an IV because I wasn't drinking fluids. I couldn't even get out of my bed and I had urinated on myself. I did experience a couple of UTI's after the vaccine. I told the doctor that I would drink broth and my son made me some. Sunday my fever went up and down and at one point. On Monday I started having a headache again and I was very nauseous. I went back to work on Wednesday with a headache. I took homemade soup and I ate there. I still had a headache throughout the day. On Thursday, I felt a lot better. A few days later, I experienced some dark vaginal bleeding, not related to my period. I had a vaginal ultrasound. I had a biopsy and everything was fine. The heavy bleeding continued and I ended up having to get an IUD to control the bleeding. I had some blood work done and my live enzymes were elevated. I had an ultrasound to check everything. I had a fibroid scan as well. I had an endoscopy. The doctor thinks I might have liver sclerosis. I told him that I do not drink. I'm meeting with my liver doctor tomorrow. There are now complications with my thyroid. I saw an endocrinologist and he said that I may need calcium in my body.

I had increased urgency to urinate. I had a lot of back pain and felt bloated as well. I wasn't sure if I had a urinary tract infection or bladder infection. Drs. office ran tests for me and everything came back ok. Heat and rest seemed to help. I was in the process of also changing some of my supplements at that time so we are not sure if that had an effect on it.

I don't know if it was related to the vaccine or not, but V-Safe recommended I report it. Anyway, I had a UTI /Bladder infection. Fever, back ache, and all the usual symptoms of a UTI.

About 5 minutes after injection, individual reported that she began to feel faint, "hot", and dizzy. Individual then had an episode of syncope. She was diaphoretic, muscles were contracted, breathing became labored, and patient urinated herself. Individual was unconscious for 2 minutes. MS services arrived just after individual regained consciousness. Upon assessment, all vital signs were within normal limits and patient reported that she no longer felt anything abnormal. Patient denied being seen by a doctor.

Started a headache about 12:00 pm progressed to brain fog unable to concentrate but still all able to manage, at about 10:30 pm I started to become nauseous, unable to form sensible sentences for about 5 minutes, extreme pain feeling like pins and needles on the entire surface of my body which pain increased with any touch, chills and rigors began and lasted for 2 hours, nausea become very bad, severe back and flank pain, abdomen felt as though insides were being squeezed, I was unable to stand due to weakness and pain, hands and feet turned purple, felt extremely dizzy, heart was racing and had some tightness in chest, ended up with 102.2 fever after taking Tylenol 45 minutes prior. I needed help to get up as I was so dizzy, weak and in so much pain, I was able to get into bathroom and at that time had pain with urination. I was able to eventually fall asleep and woke in the morning (1/21) with severe back and flank pain, pain on entire skin surface that increased with touch, fever 100.2, chills, severe nausea, extreme dizziness, weakness, exhaustion, brain fog, left foot numbness, and pain with urination. I slept most of day unable to function normally at all, did call my MD office to get a script for zofran which did help the nausea, later this evening I started to get what appeared like a heat rash across my trunk, I took Benadryl 50 mg and went to sleep. Next day (1/22) fatigue, nausea, pain was less just now low back and flanks, temp 100.1, left foot and bilateral hands numbness, ears were hot ad itchy but no rash on trunk until evening took Benadryl again. Today 1/24 dizzy, some fatigue and left foot remains numb and tingly, some low back pain

I passed out 12 hours after vaccine received while standing in the bathroom. This followed hours of terrible chills and muscle aches that I couldn?t sleep through. No medications were taken until after the episode and it didn?t happen again. I did not have a working thermometer so unsure how high my temp got. I peed myself when fainted. My husband was present to pick me up off the floor and luckily I didn?t hurt myself in the fall. I didn?t come to until he took me back to bed. I started feeling better a few hours later and much better by 24 hours. Now just lingering body aches and fatigue at 48 hours and still taking ibuprofen/Tylenol.

Had first dose of Moderna 1/4/21. Developed a urinary tract infection on 1/12/21 and started Macrobid the evening of 1/13/21. Finished last dose of Macrobid 1/17/21. Tuesday evening on 1/19/21 started with itching on my scalp. Woke up at 3:00am 1/20/21 with welts/hives around my waist and ribcage area. These hives progressed over the next 24-36 hours and I ended up covered from head to toe. I also developed a low grade fever of 100.4 on 1/21/21 and has some very slight joint swelling/tenderness. I treated the hives with of OTC Benadryl, which helped with the itching but didn't seem to have any affect on the hives. By Saturday 1//23/21 all hives were gone but itching persisted for 4-5 days after and gradually subsided. Was scheduled to get my second Moderna dose on 1/31/21 but the Pharmacist recommended I postpone for now and file a report here.

Immediately after shot: I felt some pain & burning at injection sight. In general felt a little off, but nothing significant. After observation period, on the drive home, I began to "taste" the shot, as did my husband who received vaccine right after me. Then, my face felt a little funny, and my jaw felt tight, but no visible swelling. Then I felt a rush of burning sensation throughout my chest and abdominal region. After arriving home, 20 minutes later, I felt a little nauseated, but not bad. An hour later, I was unable to eat dinner, as I felt ill. I felt like I had a mild fever and I was shivering with chills. While trying to sleep, the chills continued. I then experienced unusual frequent urination, every 2 to 3 hours. I had no consumed abnormal amounts of liquid, but my urine was nearly clear, which was odd. After returning to bed after the first time urinating, my heart rate became very fast and I felt like I had a hard time finding my pulse in my neck, which is unusual for me. The day after the vaccination I experience headache, a lot of tenderness around injection sight and arm muscle pain making it very painful to make certain movement, like putting on my top and reaching for objects. That evening I experience some muscle aches. These symptoms have improved now that it is day three after the vaccination. I still have a mild headache and pain at injection sight.

Skin feels like it?s crawling with bugs. Bodily Weakness. Headache. Alternating between hot and cold. No fever present. Sleeplessness. Increased urination.

Severe left foot & leg drop (I can barely walk), extreme left arm numbness, bladder incontinence ( I can't hold my bladder), extreme fatigue. All these symptoms lasted approx. 9 days and the left foot & leg drop did not come back to what it was before. I'd say it came back approx. 90% to how it was before.

Frequent Urination began the night of shot, urinating every 1-2 hours for 48 hours. Constant urge to urinate and slight incontinence.

PATIENT RECEIVED VACCINE AND REMAINED IN STORE FOR 15 MINUTES, THEN SHE WENT TO ANOTHER STORE. WHILE AT WALMART, SHE WAS EXPERIENCING HOT FLASHES, BLURRED VISON, STOMACH CRAMPS ACCORDING TO HER FRIEND. THE PHARMACIST SAID THE PATIENT WAS COMPLAINING OF TROUBLE BREATHING, DIZZINESS AND DIARRHEA. HE CALLED EMS AND SHE WAS TRANSPORTED TO HOSPITAL. HER FRIEND CALLED ABOUT 7:30PM THIS EVENING STATED THAT DETERMINED PATIENT HAD AN UNDERLYING UTI AND THE VACCINE EXCERBATED THE INFECTION.

Arm pain began 3:30pm Chills began ~8:00pm Awoke 12:30am with chills, body aches, tingling in R hand and lips. Temp 101.3, went back to sleep in new position to try to be more comfortable. Awoke 3:30am, stood up to use toilet. Lost bladder control, stumbled to bathroom (approx ten steps). Sat to use toilet, removed pajama bottoms. Began to feel dizzy and braced elbows on knees. Came back to consciousness leaning back over tank of toilet. Felt extremely sweaty, had to remove shirt, checked temp - 99.3. Layed on floor, vomited in garbage can. Texted family and colleagues for help in hopes someone was awake, but got no response. Decided not to call 911 because I was alone in the house with my 9yo daughter. Went back to bed and waited for sun to come up, then called my adult daughter. Took one dose Tylenol. Remained in bed until following day. Fever spiked again that night to 102.9.

Heavy and long periods occuring since 2nd dose of Moderna. I realized it on 5/9/2021...due to it being our anniversary. Experienced heavy periods/longer periods with less days in between periods. Fatigue/body aches like joint pain off and on still is continuing. Lab work turned out fine at annual dr visit.

Runny nose, fatigue, muscle spasms, hypertension, pre-diabetes, high cholesterol, body aches, shortness of breath, dizziness, frequent urination, constant coughing, dehydration, sneezing, throat pain, and chills.

9 days after second shot body covered with pimple bumps, skin rash reaction, appeared again after 6 weeks of 1st skin reaction. Same thing red pimple bumps all over body looked like chicken pox. Emailed doctor who cofirmed a reaction from Covid shot. 7 weeks after Covid shot got a bad UTI. Called doctor got meds. Did not go away after taking meds. Went in on May 25, 2021 to take a urine test at doctor. Confirmed UTI still. Given different meds. Did go away after 7 days of new meds. Did not have a menstrual cycle for 2 months after the 2nd Covid shot. Menstrual cycle resumed after 2 months. Extreme pain, huge blood clots, super heavy, lasted 10 days. It has been a month and a half and I am still not having a menstrual cycle. Menstrual cycle not the same since taking the Covid shot. Ongoing issues.

First noticed blood in urine on 1/15/21. By next day Saturday, 1/16/21, patient states it was a dark "Coca cola color" urine in toilet after urinating. Sunday, 1/17/21, urine was lighter in color. Monday 1/18/21, urine looked clear in color but test results showed that blood was still present in urine.

After shot one ( left arm) on 01/08/2021 I had severe joint and muscle pain, fevers, chills, exhaustion, nausea and shooting pain down my left arm and up to my neck and back of skull from afternoon of injection on Friday to Sunday night. I slept all weekend. After shot two (right arm) on 02/05/21 I had all of those same reactions by early Friday afternoon. I woke up at 10:00 pm with the need to vomit. While sitting on the edge of my bath tub waiting to vomit, I fainted and fell backwards into the bathtub, hitting me head and wrenching my neck as I was wedged into the tub sideways. My son found me and roused me. I then urinated all over myself, had extreme diarrhea and vomiting x 2 rounds. I fainted a second time while sitting on the toilet with diarrhea and vomited 2x more into my bathtub.

I woke up with a fever of 102.9. Throughout the day, the fever increased to 104.2. My legs were very sore which made it difficult to walk. I was extremely tired and and my left kidney area was very painful! I did not have a headache like the first injection but was very dizzy. It lasted about 30 hours and then my fever broke.

March 11: moderate arm pain March 12: diffuse abdominal pain first in the morning then centralized in the bladder/pelvic region. Iced the area and could not move without extreme pain March 13: continued bladder pain some pain with urination, fever chills, headache, some nausea, muscle pain, fatigue, some joint pain. Had an online doctor visit. Prescribed antibiotics for a bladder infection and was told to report this to the CDC.

Per her father on 5/2/21 she had kidney and liver failure and was admitted to the hospital. Was notified on 5/15/21 by her mother that she passed away. They are performing an autopsy to determine the cause.

I have not had my period since I got fully vaccinated in April 2021. And I had one trip to the emergency room because I was unable to urinate for many hours and I didn?t have a UTI

By 2:00 on 2/16/21 member had severe chills, by 4:00 a.m. fever of 101, body aches all over, severe headache from back of neck to top of head, in bed all day Sat with 102 fever; Sunday diahhrea and body aches and headach; Monday body aches and headache and also today. Chest pain, jaw pain, headache, kidney pain,. chest pain every 5-7 breaths, pressure on her chest, Pain was in the center of her chest. Jaw pain was bilateral and headache was on the left and unrelieved with advil. She also reported difficulty thinking.

1/21/21 Vaccine given at 4:30pm CST. By 11:30pm, felt feverish, started chills and headache. 1/22/21-3:30am- body and joint aches started and chills worsened, headache continued 6:30amCST-chills continued, body and joint pain worse. Took 2 Excedrin to help. 10:30am- fever of 100.3 noted. Continued with chills. Arm visibly red and sore. Swelling started in arm at injection site. 12:00pm- dizziness started. Tunnel vision, and almost passed out. 1:30pm- dizziness worsened, and weakness started in legs. 2:30pm unable to stand without assistance. Left work because unable to continue working and called the doctor. Went home, and started to drink electrolyte supplements. 5:30pm- fever went up to 102.5. Unable to stand without assistance and experienced extreme dizziness. Frequent urination at that time. Arm developed a very large swelling about the size of an egg at the injection site, and was very red. Glands under the arm started swelling, and experienced pain if I tried to lower my arm to my side. Chills continued, joint/muscle/bone pain continued. Became nauseated from the dizzyness. Took 2 additional Excedrin for fever and pain. Could not sleep due to the pain in my joints and bones, and pain under arm from glands. 1/23/21 3:30am- Frequent urination during the night. Finally regained ability to stand on my legs, and dizziness started to lessen. 8am Remained on the couch all day except to use bathroom. Fever reduced to 100.1. Continued to use Excedrin as needed for joint/muscle/bone pain. Arm too sore to touch, and swelling protruding from muscle size of a large egg. Very red, hot and itchy as well as pain. 1/24/21- A.M.- most of the dizziness subsided, with just occasional issues. Temperature went back to normal. Still severe pain at injection site with swelling, redness, and itching. Some joint/muscle pain still. 1/25/21- Mostly Arm/injection site issues. Still red/painful, swollen, and itching. very mild joint/muscle pain. 1/26/21- Arm/injection site issues more mild. Swelling reducing, pain & itching reducing. Glands under arm reduced, and pain reducing. 1/27/21- Slight redness at injection site. Swelling down to size of a grape. Slight pain & Itching. Glandular swelling & Pain completely gone. 1/28/21- A.M- No symptoms at all. By 9am, started having twitching/muscle spasms on Left side of scalp above ear. Progressed and worsened through day. By 4pm, spasms and twitching lasts bursts of 2-3 minutes. then stop for 15-20 seconds. Consulted with physician, and concerned about vaccine related issues. 2/5/2021 at 1:30am sharp headache woke me up, 6:30 freezing cold, achy joints, red rash warm hard painful at injection site 9:00am achy muscle, joint pain, fever 100.5, freezing cold and shaky continued entire day. 2/6/2021 2am kidney pain, muscle pain, headache no longer cold, no longer had fever. Warm, hard painful red rash at injection site about 4 inch rectangular shape increased in size 2/7/2021 red warm hard painful rash at injection site and headache

received the second covid injection 2/3/2021 at about 1pm. The typical symptoms began with soreness at injection site. Then random joint pain, muscle aches and stiffness. During the night muscle ache turned to pain radiating around upper abdomen up into right shoulder as did the injection site with nausea, stomach upset and gas. 2/4/2021 around 9am the chills started and fatigue, increased muscle aches and chills with difficulty controlling my bladder, in addition my urine turned dark. 2/5/2021 all the same symptoms with increased discomfort around upper abdomen circling to my back and new muscle twitching at the injection site. All continued through 2/7/2021 and began lessening. Starting 2/8/2021 mild aches and random chills with upper abdominal pain continuing. At night for some reason it all starts again, I wake up wet from perspiration every night and I switch from painful chills with aches and pain to being hot.

That night, headaches and chills really bad. By that evening, around 6, I couldn't get off the couch by myself. I got up and it felt like my ribs were rubbing against my kidneys. It was hurting that bad. I felt like I did when I had COVID the first time. I just laid on the couch. I took 2 Ibuprofen that helped more than Tylenol. My lymph nodes were swollen and I couldn't even put my bra on. The swelling was also in my neck. I continued to have chills and a fever. I was sweaty. From Friday to Sunday, it started to dissipate and then I was able to get around better. My lymph nodes were swollen for about 5 days. I started to feel better, with the exception of my arm which is still bothering me. I experienced sharp pains in my arm last night and it's been itchy.

Within the first 24 hours of getting the shot I developed some tingling in my right arm and hand, and right leg and foot. Also pain between my shoulder blades. This subsided after two days. Beginning 26 hours after the shot I had stomach cramping, nausea, extreme fatigue, body aches, headache, chills. This lasted for 3 1/2 days. Then I developed a severe cystitis flare up, which I had not had in years. I?m still dealing with the cystitis flare which causes fatigue, bodyaches, frequent urination, insomnia, pressure in the bladder and pelvic area, extreme discomfort, nausea and headache. I?m taking 200 mg Advil every four hours, using a heating pad, drinking mint tea, and taking desert harvest freeze dried Aloe Vera capsules (6) daily.

Pt. spouse - Reported today (3/12/21) pt. is in the hospital for "kidney failure". Pt. spouse said she was feeling nauseated on 3/4/21 and then later was in the hospital for treatment of "kidney failure".

Patient developed urinary retention the morning after receiving the vaccine. Inability to void without valsalva, small void w valsalva, post void urinary residual, bladder distension (mild). No dysuria or hematuria. No hx similar.

Approximately 1.5 hours after 2nd Moderna vaccine I developed a full body rash that ranged down the inside of both arms, the front/back or torso and down the inside of both legs. It was a spotty red rash that did not have raised bumps and it did not itch. After 2-3 days the rash no longer exists on arms or torso but it is now 7 days after vaccine date and I still have the rash on my legs.

Began with tingling down my right arm, and I became a little shaky. Drank some water and the shakiness continued to get worse. Went to the restroom and felt like I was going to pass out. Tingling came back across my upper body (chest level to shoulder level), I began to shake excessively, my mouth got very dry and my skin became splotchy. I was breathing hard like I had just ran flights of stairs. Nurses were called and I was given Benadryl and taken to the Emergency Room at a neighboring hospital where they hooked me up to I believe heart and BP monitors. My blood pressure was very high and my heart was pounding so hard. Was given steroid pills. All day I urinated excessively and my heart pounded hard.

Woke up at 1:50AM due to bilateral kidney pain. Also had nausea and sore arm upon waking. Within an hour, at approximately 2:30AM, all skin became very sensitive/painful to any clothing/contact, also got chills, low back/hip/knee pain, and continued nausea and sore arm. Took 2 extra strength Tylenol (500mg each) around 4:00AM. Few hours later skin sensitivity and chills greatly diminished around 5:30AM. Continued with aching pain in lower back, hips, knees, and feet (feet when walking). Also continued with nausea, sore arm, and minor skin sensitivity. At 8:30AM, still experiencing aching/painful back/hips/knees/feet, nausea, and increased skin sensitivity/pain.

At 6pm on 1/25/21, 10 hours after dose 2 of the Covid vaccine she began to shake, have chills . She did not take her temperature but felt she had a fever. She went to sleep and woke at 4am and had wet the bed. The next day, 1/26/21 she had a headache, tremors and chills. She stayed home from work. On 1/27/21 she went to work, felt better except for a headache. On 1/28/21 she went to work but felt "weird". At 12:30 that day her head was pounding, glassy eyed, felt chest pressure and was "foggy". She went to eat lunch but on the way back to the elevator her legs buckled and she could not walk. She was escorted by wheelchair to her desk . Her Manager stated she did not look well and 911 was called. She was taken to an adult ED. She was thoroughly evaluated ( Head CT, blood work, EKG ) give IV fluids. She was found to have a UTI and was placed on Keflex. Given RX for nausea and vomiting. She was discharged home to follow up with PCP. 1/29/21, she is feeling better except for some fatigue.

I became very chilled, could not get warm yet was hot to touch, had a fever close to 102, shortness of breath and coughing, urinated approximately 10 times in 2 hours, nausea, blood pressure dropped and heart rate went up (typically heart rate runs around 60, went up to 80-83 consistently, fatigued, ached all over. This started the evening of the shot, the severity of the symptoms lasted throughout the entire next day, and the fatigue, nausea and achiness is continuing still today. The first night I thought I was going to have to go the ER, but finally got a little sleep and fever went down by morning.

Post vaccination I had severe joint pain and swelling. This is still occurring and it is now 2/10/21. 3 days after vaccination I also developed UTI symptoms. Those symptoms increased for 2 days and I sought medical attention. There is no other reason for me to have developed a UTI. Another co-worker that had the vaccination the day prior had the same symptoms.

7/4/2021: Patient transferred from Hosptial after presenting with generalized weakness, cough, shortness of breath and blood in the urine. Patient tested positive for COVID on 7/3/2021. Diagnosed with acute bilateral pneumonia secondary to COVID-19, acute hypoxic resp failure, severe ARDS, acute metabolic encephalopathy, AKI. intubated in the ICU. Note: patient fully vaccinated with Moderna COVID-19 vaccine. 7.25.21: patient died.

Received first dose of vaccine on 12/29/20. Had no reactions and checked in through Vsafe system. On 12/31/20, I lost control of my bladder at work. I did not have the urgency to go, and when I started walking, my bladder emptied. I reported this symptom through Vsafe on that day. This symptom did not happen again. On 1/10/21, I was changing my clothes and my left arm felt itchy. When I itched my arm, I felt little, raised welts. I looked at my arm, and a significant red, irregular shaped spot was on my arm. I could not report this to Vsafe as my check ins had stopped as of 1/6/21. I called my PCP, who directed me to this site to report it.

Patient had first dose of Moderna vaccine at another facility on 1/10/2021. Around 1/18/2021 she felt poorly, more fatigue and noted her urine was dark. By 1/22/2021 she opted to present to the ER where she was noted to have severe hepatitis (ALT 1600) but oddly a normal bilirubin. Her INR, CBC and vitals were normal. She was sent home from the ER and I saw her today 1/25/2021 in consultation for the liver enzyme issues. It is not clear that this is related to the vaccine. She may have hepatitis A or this could have been a medication/supplement induced hepatitis but due to the timing I felt it should be reported.

Sepsis, Acute Pancreatitis, Respiratory Failure on Mechanical Ventilation, Disseminated Intravascular Coagulation, Pneumonia, Acute Kidney Injury. Refractory Hypoglycemia

Blood noted in urine, flank and urethral pain similar to previous experience with kidney stones in the past (over 15 years prior), no visual stone observed. Fever=101.5, chilling on 1/28/21. 2nd dose of Moderna received 1/26/21. Typical local reaction of redness, swelling, pain at injection site. HA, fatigue, body aches.

Vaccination 1/29/21 - Slightly tired 24hrs after injection; nausea and diarrhea 36hrs after injection until Tuesday afternoon, February 2nd; slight nausea and some diarrhea through the date/time of this report; UTI diagnosed evening February 3rd

2/3/21 Started with headache. Then overnight fever and chills. Dizzy, body ache, frequent urination. Next morning 2/4/21, fever, headache, upset stomach, body aches, fatigue. I had to quit work and go to bed. 2/5/21 fever has passed. Still have headache and some body aches.

2/3/21 Started with headache. Fever overnight and chills, body aches and frequent urination. Took Tylenol. 2/4/21 fever increased, headache and body aches got worse. Extreme fatigue. Took Tylenol and ibuprofen. I had to quit work and go to bed. Normal sleep is about 5 hours. I slept from 3pm to 7am. 2/5/21 I am feeling somewhat better, still have body aches but am able to eat and fever seems to be back to normal.

01/22/2021- dizziness, weakness, palpitations and tachycardia still reoccurs until present date. 01/24/2021-developed swollen lymph nodes on jaws, neck, above collarbones and shoulders still present to this date. 0128/2021-very painful swollen left shoulder and above left clavicle with pain extending towards upper back to both scapula prompting urgent care visit where high BP and heart rate were also elevated @199/133 and >100 respectively. Amlodipine 5 mgs PO daily started.

On day 1-2: severe fatigue and lethargy. On day 10: sudden nighttime urinary incontinence - this has never happened before but both my husband and I have both now had this side effect

She reports history of Moderna vaccination on Friday, March 5th. It was her second dose. She felt awful within an hour of vaccination and persisted with symptoms for 2.5 days. She described it as being out of it, headache, fever, chills, profound body aches, weakness and dizziness with palpitations. She could not get out of bed for a day. She literally thought she was going to die given the severity of her weakness. As quickly as the symptoms occurred, they left her on March 7th, 2021. She went to work on March 8th and noted similar symptoms to prior ITP flares - petechiae on legs, gum bleeding, fatigue, mild headache. She also noted dark urine but no frank blood. Since being admitted and receiving steroids, she is feeling improved and petechiae are lessening. She also noted mild mouth ulceration to lower lip along with oozing.

Herpes outbreak that turned into viral meningitis and severe UTI. 9 days in the hospital on IV medications. Transported by ambulance to hospital in, 3 hours away and out of state. Initial symptoms were vaginal burning/itching followed by rash. Low grade fever, inability to urinate. Finally, severe, debilitating headache, stiff neck, severe pain between my shoulder blades.

3/23 10:40 am felt hot all over. Continued 36 hrs, and off and on since then. 3/23 11:00 am very tired. Unresolved but improving at this time. 3/23 11:30 am urinating unusually large amounts. Continued 48 hrs in large amounts before import. Became dehydrated. Urination became normal again 3/27. 3/24 evening nausea, diarrhea lost appetite, chills low grade fever (99.2), heart pounding. Fever, diarrhea, heart pounding and chills ended morning of 3/25. Nausea ended evening of 3/25. Lost appetite continued through 3/28.

I ended up in an unspecified hospital with what the doctors thought was a UTI but it traveled to my kidneys. A few weeks later I started to have issues with my thryoid disease and went in for a battery of tests. My immune system was on overdrive and everything was off with my blood work.

10 minutes after administration, patient reported feeling "woozy." She then lost consciousness and urinated while unconscious. She then regained consciousness after 5 minutes. No other symptoms except increased sweating. Blood pressure was taken by paramedics and would not go above 70/50.

Two weeks after the vaccine, I had a sudden onset of abdominal pain on the left-side. I also felt queasy. The pain was very sharp, like there was a knife in my side. I went to the emergency room. I was given an IV with pain medication and a CT scan. I was given contrast, so they were unable to see if I had a kidney stone. The pain was better. I was discharged after three hours. I was able to manage the pain myself with over-the-counter painkillers. It lasted for a couple of days. The next day, I felt pain in my right flank and it was moving south. After speaking to a doctor again they thought I had a kidney stone on both sides. I drank a lot of water and took some Advil. The pain went away after two days. I have never had kidney stones before I got the vaccine.

Multiple blood clots in lungs, and kidney fuction has declined . Was in hospital for 3 days for treatment. I am currently taking xerelto for clots.

Next day -sore arm. By evening, felt flu like symptoms and cystitis set in Called in sick that Monday and Tuesday. One week prior to my second shot, I was diagnosed with an UTI. Since my last was 2008 and nothing had changed except the first Covid shot, I thought it was note worthy.

After vaccination in the evening patient has a mild complaint of throat burning. Patient went to bed and awoke at 4AM with severe arm pain which was managed with an ice pack. Patient was able to sleep for about an hour, awoke with headache, body aches, sore kidneys, and a queasy stomach. Also complain of metallic taste on her tongue. Patient has taken tylenol and cough suppressants for symptoms. Had similar symptoms after first vaccine, but these are worse. Patient likely would not have gone to work except for strict attendance policy.

With both doses I had high fever, chills, nausea, vomiting, headache, body aches lasting three days. I also urinated blood for three days. My creatinine levels more than doubled from last taken date of 11/2019 and my GFR was very high. When this happened with the second dose my doctor ordered a CT scan and sent me to a Nephrologist, who ordered a kidney biopsy.

3/29/21 -one hour after arriving home from getting injection, felt fatigued, took 1 1/2 hour nap, a headache occurred that evening 3/30/21 - All during the early morning hours after midnight, had headache, restlessness, could not sleep, had diarrhea twice and frequent urination. After waking at 10:00 a.m., had severe headache, pain on top of scalp, tender forehead, constant fatigue, chills for three hours, severe stomach cramps and diarrhea for two hours, light headedness throughout the day, slept on and off all day. While eating dinner that night, realized my tongue felt scalded and sense of taste was decreased, blah. 3/31/21 -Woke up feeling better, headache was gone, made breakfast, ate and still had decreased taste sensation on tongue. Continued to shower, dress, wash dishes, then felt fatigued and light headed and more diarrhea after three hours from waking, laid back down and took three hour nap. Took it easy the rest of the day, feeling slight headache and sinus congestion.

Patient complained about sore arm after injection. About 2 hours patient started getting body aches, fever, chills and sleepy. She said slept most of 2 days. Sometime the first 24 hours she develops diarrhea that was still an issue 4 days later. She also reports she developed a "Bladder" infection with burning and pain about 4 days after injection. She did have her husband buy phenazopyridine 95mg tabs for the bladder pain.

Icy cold sensations in arms and hands for 45 min, some in my back. 3 hours later, a heavy fatigue, pain in legs began in random places, throbbing, pain in fingers, down both arms, in bones in hands and ankles. Approximately 48 hours after my second Covid vaccine, I spiked a fever of 101.5 and had extreme kidney pain. Frequent urge to urinate. I ended up going into the doctor to have a urine test checking for a bladder infection. Blood was found in my urine. My doctor put me on an antibiotic. I have not had a bladder infection in over 20 years and I have never had kidney pain.

Approximately 8 hours after receiving vaccine on 6/3/21, pt experienced body aches, weakness, loss of bladder control, vomiting, severe cough, throat swelling, loss of appetite, crushing chest pain, severe arm pain that limited motion, and fever. After patient was administered Moderna COVID-19 vaccine, patient came into observation room feeling very nervous. At 4:55 PM, patient complained of intense itching on her face and scalp. This was accompanied by flushing of face. However, patient reported no breathing difficulties. Patient was immediately given ice packs and applied to groin, neck, and head. Patient did not feel any relief. 50 mg of diphenhydramine was given to patient to help with intense itch. Patient was feeling better and rested for some time in observation room. At 1740, patient had difficulties standing on her own. At 1745, patient voided in her clothes at her own request. Due to medical evaluation needed, EMS was called at 1800 and arrived at 18:08. EMS had to move her from the cot to the stretcher as patient couldn't stand on her own. EMS left with patient to emergency room around 18:15

My immune system has been depressed for the past two weeks following the injection. I've gotten terrible allergies, sinus infection, bronchitis, UTI and severe asthma

Passed out; she lost control of bladder; Blood pressure low; A spontaneous report was received from a consumer concerning a 53-years-old, female patient was who received Moderna's COVID-19 Vaccine (mRNA-1273) and passed out (loss of consciousness) and lost control of bladder (urinary incontinence) and blood pressure low(hypotension). Patient's past medical history included, low blood pressure. It was normally on the lower end. She had fainted 10-15 times in her life. The patient was going through menopause and had been having hot flashes. Products known to have been used by patient with in past two weeks included bupropion hydrochloride and vitamins nos. On 02-Mar-2021, 5-7 minutes before the onset of events, the patient received their first of two planned doses of mRNA-1273 (Lot number: 010A21A or 010AZ1A illegible), intramuscularly in left arm for prophylaxis of COVID-19 infection. On 02-Mar-2021, the patient passed out. She lost control of bladder. They took her into room, her blood pressure went low and passed out, they got her blood pressure back up she came around in a couple minutes. They called 911 but no ambulance came. When the patient felt good enough to leave, then they sent them home. She felt effects for rest of day, but felt normal the next day. No treatment information was provided. Action taken with mRNA-1273 in response to the events was unknown. 9 pm fever went in 15 minutes to 101, 102 , 103.4 by 10 pm 106.4 Packed neck with ice packs to protect my brain. Fever down to 102.3 but by 2.30 back to 108.8 degrees, Muscle loss, pain in joints, extreme fatigue, shivering, chills, front lob of head hurt, can't function by myself. New ice packs around my neck, cold compress on my head, 45 minutes later temperature down to 102.3. Next morning up to urinate, blood in urine, urine was clear however could see red. Next day only up 3 times for 15 minutes each. Forced myself to drink water and Gatorade all day. Use wall to walk, unable to do any daily activities, hurt all over, so weak. Temperature throughout day low 101 with various highs 104.7, 105.2. No energy, no longer in control of my body. 4.30 am. Sunday 4/11/2021 woke up soaking/drenched wet. Sat up muscle control is back, pain is gone. Long 31.5 Hours!

After both my first and second shots I became incredibly thirsty. I needed to pee more than usual and could not stay hydrated. This morning I nearly fainted and think it was due to dehydration. I also ran a low fever, had chills, body aches and a very strong headache. But the thirst seems unusual.

Patient developed a vasculitic rash on her lower extremities approximately 36 hours after receiving the moderna COVID-19 vaccination. She also had a fever, bilateral ankle pain, malaise. The patient was also on nitrofurantoin for a urinary tract infection. The patient was hospitalized overnight. Skin biopsy showed leukocytoclastic vasculitis. In my opinion this was more likely related to the nitrofurantoin than the COVID-19 vaccination, although that possibility remains. Care discussed with a dermatologist who agreed. There is no test or way to sort out the causative agent.

After my second vaccine on 4/22/21, I woke up in the middle of the night with fever, chills and my RIGHT shoulder had a severe stabbing pain that would not stop. I also woke up to a UTI infection as well. The fever and chills lasted for 24 hours and went away. The right shoulder pain continued for days. By 4/29 the pain was too severe to tolerate. At this point I could no longer move my arm at all and it was hot to touch and too painful to touch. I went to the ER 4/30 to see if they could help me with the pain. They X-ray my shoulder, gave me a steroid shot to try to reduce the inflammation, a valium, toradol, and lidocaine patch for the pain. They gave me the name of an orthopedic to see for a cortisone shot in my shoulder to reduce the inflammation. I received about 15 hour relief from pain. I saw Dr., orthopedic on Monday for my shoulder. I gave her the timeline of information. She did an X-ray as well. She gave me a cortisone shot in the back of the shoulder. Within an hour I could immediately feel some relief. By the next day I was able to move my arm some and not have the constant pain. I have a follow up in 1 week.

Pt reported that after dose # 1 she had "dark urine" for a few days that resolved. Dose #2 administered 4/5/21. She developed profound fatigue, dark urine, shortness of breath and presented to her PCP 4/17/21. . Had chest CT to r/o PE. Negative. Labs revealed AIHA. Admitted to hospital 4/18/21. Discharged 4/25/21 with close OP follow up.

53 year old female with hypertension and recent COVID-19 vaccine (Moderna, 1st dose on 4/5/21), sent for abnormal labs to the ED and new onset dizziness. As for the dizziness, the patient describes it as: of 2-3 week duration, onset 4 days following COVID-19 vaccine (Moderna, 1st dose on 4/5/21), progressive, intermittent, reproducible with activity, some improvement at rest, of variable pain intensity; associated with taste change, dry mouth, malaise, nausea, and poor appetite. Of note, urgent care reported BUN 54 and Creatinine 2.89. On arrival to the ED, the patient's CT of head was notable for: No evidence of acute intracranial pathology

Starting around 9 PM, I had chills and felt my temperature going up. By 10:30, I had more than full body chills. They were like full body spasms that made it hard to breathe. My fever went to 103 degrees. I tried to sleep, but it was extremely difficult. I did finally sleep, I woke up and I had difficulty managing my body, hands, and brain. I lost bladder control. I took my temperature and it was 104. Went back to bed with difficulty. I woke up a couple more times with the same result. However, I didn't have the brain power to take my temperature again. I could barely find the light switches and almost went into the closet instead of the bathroom. (That's what my husband told me) 8 or 9 am, my fever was down to 102 degrees, my head felt like someone was splitting it open and my body felt like I had bruises from head to toe. I was awake MAYBE an hour all day that day. My lungs were on fire and I was short of breath again. It was like I was recovering from COVID pneumonia again. My doctor prescribed prednisone for my lungs. My right arm had a reaction below the injection site. It was swollen and hot. My left arm that had gotten the first injection also started hurting again. It was also swollen and hot.

Arm turned red at injection site and hot. There was really no pain and went away within a couple of day but turned red again exactly a week later from injection date. This time it was hard, hot, red, itchy, and bigger. Dr treated it with clindamycin and dose pack of steroids. It got better after a few days. Next, I got a UTI and called my primary care physician. She ordered CIPRO and phenazopyridine. That got better but before CIPRO was finished, I got a crick in my neck that turned into excruciating pain in my neck and was unable to turn head from side to side. At the same time, my left foot became painful, red and swelled. I have been treated for arthritis in that foot 6 years ago with an injection. I have not had any problems with it since the injection. Dr. treated my neck problem with Robaxin and a steroid dose pack. This seemed to help my foot too temporarily. Neck pain improved but not gone. It was excruciating pain up the back of my scalp at times. I can turn my head some now but it's not back normal. Can't turn it all the way. Dr. did another injection in my foot but did not work this time. I got worse and I can no longer put any weight on that foot. I have not been able to work since January 20. Have had to apply for leave of absence and short-term disability

Ten hours after vaccine I experienced a hyperglycemic event; my glucose was so high than neither my arm sensor (Free Style Libre) or my finger stick meter could read it. It took 40 units of Humolog over a 2 - 1/2 hour time period to bring it down enough to read it. It was 452. By morning, six hours later and 20 more units, I am down to 236. My systems started with my heart pounding out of my ears and neck, frequent urination, and sweating or feeling warm (usually associated with low blood sugar events). I ate lunch after the vaccine, but by evening when I would normally make some dinner, I didn't eat until I forced myself to eat some cooked cauliflower at 12:30 a.m. As I was finally going to bed, around 2 p.m., the cough started. The cough is an itch at the base of my throat with a slight feeling of swelling. I have sugar free cough drops and cinnamon candy (which helps reduce the swelling). This morning, four hours later, the cough has a small amount of phlegm with it (but mostly just itches).

Swollen lymph nodes in left axillary the size of a tennis ball for about 4 days but not completely subsided after 2 months. Painful kidneys for 4 days followed but low back pain for 3 days.

Arm sore, Within hours Burning when urinating, very low energy and head felt foggy. Day five started severe chills and sweats and low grade fever. Nausea, faint, diarrhea. Day 9 Glands in neck swollen hard to swallow. Day 13 back to the Dr, still have all symptoms. the severe chills are the only symptom that is better.

After feeling sick for 24 hours, I started urinating bright red blood. It went away about 48 hours after it started.

The patient receive dose 2 of the Moderna vaccine on March 31 at approximately 9:30am. Began having symptoms of low grade fever, severe malaise, nausea with vomiting, and body aches about 5:30 that evening. The patient presents to the Emergency room on 04/01/2021 at about 3:30pm with skin redness, back pain, nausea,vomiting, low blood pressure and tachycardia.

pt says she had sore right arm that was hard to lift for about 3 days. She had shaking and chills along with nausea that lasted about a half an hour. On 4/25/2021 she woke up feeling bad with kidney pain in her back. She couldn't get comfortable. She developed headache, nausea, stiff neck and joint pain. She had head pain, not like a headache and feeling of being cold. Her right arm became itchy and the pain returned like she had just received the shot. On 4/27/2021 her upper right arm has a itchy red rash on it, she has a slight temp, headache and her tongue is burning. She is only taking Tylenol for her symptoms.

Patient received vaccine April 2, 2021 and initially had low-grade fever up to 100 and myalgias, mild symptoms. However, patient then had intermittent shortness of breath for the next few days and on April 12, 2021 developed wheezing and shortness of breath. Patient presented to the clinic on April 13 due to significantly worsening respiratory distress. She was seen at our affiliated walk-in Center. She was given Benadryl and IM steroids but then went on to develop swelling of the face and vocal cords. EMS was called. Patient was transported to a local emergency room bayonet point where she was admitted, given more steroids and nebulizers, however, patient ended up intubated and transported to the intensive care unit for the next 4 days. On exam, it was noted patient had subglottic angioedema. Patient does have a history of asthma and allergic reactions

abdominal pain, hematuria, nausea, vomiting for 2 weeks. Has had 4 ED visits and continued pain despite negative workup.

Hematuria/blood in urine started the day following the 2nd dose of Moderna injection. Has persisted for 3 1/2 months

Around the 8th of March I had intestinal problems. I could not eat, walk, diarreha, dizziness, balance issues, dry heaves, overheating. Doctor said it sound like a viral gastroenteritis. About mid April, I had muscle weakness, fatigue, overheating, could not care for myself. I was very weak. In between episodes I get little thing happens, my skin burning, circles on my back, shocks that go down the sides of my feet, and numbness in my feet. I was referred to a neurologist. I also had a UTI.

I do not get UTIs. I came down with a very strong and bad unitary tract infection. I was put on antibiotics to clear it up. I had my second shot and have another UTI. This is NOT common for me. I haven?t had a UTI in over 25 years.

Developed immediate fever & cills within 2 hours. By next morning had pain & difficulty producing urine. Saw primary care doctor and urgent care center, infection not treated w/PO Macrobid after 2 days. Admitted to Hospital w/sepsis, kidney infection and acute renal injury?creatinine 4.25 (baseline=1.5)

On the third day I got a nasty smell in my urine. I went to the Doctor and they told me I started to get an UTI, she gave me medicine. She told me the smell, I would have to drink a lot of water. She told me the smell may have been from t he vaccine but she couldn't tell me for sure.

I had a uti and was prescribed an antibiotic Nitrofurantoin on 1/23/2021 -didn't start it until the next day - finished the antibiotic - 1/28/2021. On 2/1/2021 I started to feel sick, I had chills, temp 99.5-99.7 rash - at first just around hips, buttocks lower abdomen - areas of erythema that was raised and indurated largest area was about 8 cm diameter, very itchy, later that day swollen hands, feel, large tender lumps that were normal skin color. A few hours later I developed macular rash to top of my shoulders, itchy and tender - looked and left like a sunburn. ( but no sun exposure !) My wrists , hands, elbows were sore. As the rash on my lower torso improved I had a second outbreak abdomen , I took benadryl 25-50 mg every 8 hours. I contacted my employee occupational health - discussed with them - at the time I was concerned about whether they wanted me to be tested for covid - I am a healthcare worker . At the time we both thought may be it was the nitrofurantoin. I also asked about whether I should hold on my 2nd vaccine- but by this time rash was much better, chills were gone, joint pain and swelling improved. Occupational health recommended I get 2nd vaccine. On 2/4/2021 I got 2nd vaccine, no new symptoms that day. Next day 102.5 fever but rash was almost completely gone and no longer itchy. On 2/6 .2021 rash came back but much worse, same symptoms , rash more wide spread, hive like rash with white border pink in center, various sizes but including breasts , thighs, buttocks abdomen, and starting on my arms but no joint pain. I was taking benadryl 50 mg every 4 hours without much relief and pepcid 20 mg. This morning 2/7/2021 I developed facial swelling , lip swelling, - started on prednisone, now things improving

Walking to bed and headed to restroom. I felt perfectly fine. Blacked out in the restroom. Hit chest on either door handle or toilet. bruised ne and lost bladder control. When i awoke I had chills and fever. Crawled to bed and took temp. 102. Shivering and chills all night. Fever stayed at 102 for evening. Next day it was 101.5 all day. spent day sleeping and through the next night. Chest pain from hitting chest. exhausted entire weekend ..

Patient is a 55 y.o. female presenting from home with complaint of shortness of breath. Patient tested positive for COVID-19 infection in January 15. Last 2 days started to get sob with mild cough, no phlegm production Denies chest pain Initially had fever, but no fever last 3 days In ED, dropped sat to 89% momentarily, but was found to be tachycardic 130 range sinus CT PE was negative for PE, showed Mild infiltrative changes of the left lower lobe and lingula. Likely related to patient's known atypical infectious process. No superimposed dense airspace consolidation Pt has been treated with Remdesivir, Decadron and supportive care. Pt has been stable in room air. Pt wants to go home today. She will receive the fourth dose of Remdesivir. Then she is OK to be discharged home. She will follow up with primary care physician Pt reported dysuria. Her UA showed positive nitrite. One dose of Cipro 500 mg given (pt is allergy to Amoxicillin). She will continue take Cipro 500 mg twice a day for 7 days for UTI.

Evening of 2nd vaccination - uncontrollable chills/tremors/shakes, fever with burning eyes & ears, took 2 tylenol, 3am (01/27) heavy blood in urine, went back to My Urgent Care so they could record side effects, they said not side effects but possible UTI although culture was negative, prescribed 5 days of Keflex, 01/27 blood pressure high and incredible headache started in the evening, blood in urine gone by Sunday 01/31, televisit with primary care on 02/03 for sudden change in BP and discuss the "UTI", Primary care prescribed a second dose of losartin, by Sunday, 02/07 - BP so elevated, trip to ER and blood back in urine, ER did CT Scan to make sure no kidney stones, negative, and bladder/kidneys look fine, however, still have IUD, so may want it taken out since post menopausal, 02/08 in office visit with primary care, administered clonidine to get BP down because was up again, he had me do 24 hr urine collection, said go ahead and have IUD taken out, but also see a nephrologist as well as prescribed clonidine as need for BP issues, since that visit BP has seemed to regulate with a few spikes requiring clonidine, IUD removed 03/09 and will see nephrologist on 03/15, when I saw Gynecologist, she mentioned a doctor's wife who had similar issues with Moderna, and that I should report

Sore arm on first day, but woke up with very dark and/or blood-tinged urine 36 hours later (today). Back pain and some aches, not sure if related to kidneys. Has persisted all day.

Extreme muscle pain following day after injection. Unable to move, so painful thought out was passing a kidney stone. Husband called ambulance. CT scan revealed stone still in kidney, pain over left flank due to muscle spasm, most likely cause Moderna 2 shot. No activities that could of strained the area. Pain gradually subsiding as pain in deltoid also subsiding over last 3 days.

Patient received 2nd dose of Moderna COVID vaccine during the day. Patient had soreness at injection site but no other symptoms. That evening woke up with extreme thirst, drank multiple glasses of water but did not void. The next day had tingling sensation in the face and body, then generalized flushing of the body. Within hours patient had emesis, diarrhea, watery eyes and palpitations. Had telehealth visit and prescribed steroids.

Was in hospital til Saturday when she received her Moderna Vaccine 1 . That evening, had low grade temperature, fatigue, body aches. On Sunday, she took her Humira which she normally takes every two weeks. On Monday morning, started to notice that she had oral ulcers as well as cuts on side of mouth. Tongue felt like it was really dry. Mouth was really painful. Went to ER at who thought that this was oral candidiasis versus oral psoriasis. Suggested she get B12 checked and also start nystatin swishes. On Tuesday started to notice that her hands were peeling and having much pain. Used triamcinolone cream on both of her hands but this is not helping. Use of lidocaine for pain only causes her hands to burn more. Husband bought her a pair of cotton gloves to wear.

VIolent shaking for 2 hrs, horrible headache, fever, nausea, generalized skin pain, hair follicle pain, diarrhea, cramping, abdominal pain, face rash on left side and swelling in face, dizziness, abdominal distended, kidney pain, frequent urination, loss of appetite, bones felt frozen. And no this isn't conversion disorder.

About 7 hours after the 2nd shot I started having chills and body aches. This progressed to a fever of 102, severe headache, chills, body aches throughout the night. The next day bad flu symptoms persisted. Slept most of the day - totally exhausted couldn't keep my eyes open. Severe kidney pain and urine was a reddish/orange color. Made sure to drink plenty of fluids. Also developed hive like rash on lower back. Within 48 hours symptoms ceased. Only rash remained which lasted about a week.

Uti subsequent to both shots...also had fever and nausea after second one. Was fine after first. Did have Covid in February

A four-fold increase in Urine Microalbumin/Creatinine 10 weeks (on 4/23/21) after second Covid vaccine dose (on 2/10/21).

Around 2AM I was sitting at work and I looked at my co-worker and told her my head was killing me. I took Advil, it was like the worst "flu shot" I ever got. Then the muscle pain started. I went home and took some Advil, tried to walk my dogs and couldn't. I usually walk 3 miles with them and did not walk even 1/2 mile. Went home and took a hot shower with Epson salt and laid down. My arm started aching. I can't even wash my hair, the injection site itches and is painful and my scalp was itching too. Overall headache, joint/ muscle pain like flu like symptoms. Wednesday morning before I left work my back started hurting really bad, severe back pain. When I arrived home and used the bathroom I was having abd pain, and diarrhea. It was all fluid. I ate some chili the day before and when I wiped my bottom it was red like bloody. My PCP was closed. It happened about 4 times. It is like I have a kidney pain (the back pain)

Hematuria noted about 1 1/2 days after receiving the first COVID vaccine. No treatment, pushed fluids and cleared up.

probable vaccine related myelitis ( seen by neurologist and physiatrist) , burning pain in legs, heaviness, difficulty walking, banding around back of spine to front, spascity, constipation, urinary frequency, Physiatrist seen an neuro exam was abnormal suggesting a myelitis ( I did not tell him I had the covid vaccination, which was an error on my part) Seen four days later by neurologist neuro exam essentially normal except for my hyperreflexia (since transverse myelitis 2003). No MRI done ( as I didn't contact him in a timely manner ) by the visit most of symptoms have resolved.

Moderna COVID-19 Vaccine EUA. I received the COVID-19 Vaccine (Moderna) shot in my upper left arm on Wednesday, Feburary 3, 2021. I had a sore arm for a couple of days and I thought that was the worst of the adverse events. But, i was feeling fine when I went to bed on Thursday night. But, when I woke up on Friday, February 5, 2021 at 8:30 am, I had this horrible pain in my lower leftside of my back in my kidney. The pain was so bad that it hurt every time that I would move, pass gas, cough, sneeze, or getting in and out of the car. I couldn't lay flat on my back to sleep . This horrible kidney pain lasted until I got up on Sunday, February 7, 2021 at 8:30 am.

Low grade fever (99) , chills, headache, right arm pain (severe) general malaise, fatigue, poor concentration, joint pain (all joints), burning eyes, kidney pain (both but more so on right side than left.

Diarrhea, acid reflux, continuing from first shot. Headaches continuing from first shot. Kidney pain and difficulty urinating (first urination after awaking) 2 days after 2nd shot. Light-headedness.

1/21-22 ARM WAS SORE AND SWOLLEN 1/23 BECAME VERY NAUSEOUS AND HEAD BECAME UNBEARABLY ITCHY 1/29 NOTICABLE BALD SPOTS ON THE CROWN OF MY HEAD AND NOTICABLE HAIR LOSS AND STILL NAUSEOUS 1/29 ALSO HAD A CT LUNG SCAN(SET UP PREVIOUS TO SHOT) AND WAS TOLD THERE WAS AN ISSUE WITH MY KIDNEY(LEFT) SCHEDULED A ULTRA SOUND FOR 2/23 2/18 SECOND SHOT 2/19 VERY NAUSEOUS AND CONTINUED HAIR LOSS(CLUMPS) 2/23 ULTRASOUND DONE AND FOUND THAT LEFT KIDNEY HAD FAILED.

Fitful rest night of vaccine. Day following :Fever 101, extreme muscle cramps, body aches, back pain, brown urine, extreme fatigue, headache, light sensitivity, nausea, weakness, cold extremities, chest tightness. Day 2 decrease of mentioned symptoms. Day 3 near normal, fatigued, continued cold extremities, mild headache. No treatment or tylenol taken as I was under the impression I wasn't suppose to suppress immune response. I was well hydrated prior to and during response. Had COVID-19 in July. I will not get second dose secondary to reaction of the first dose.

Started with high fever on Sat (101), extreme weakness on Sunday, Monday and Tuesday. Went back to work on Wednesday, still not feeling but felt I should be trying to do something by then. Drove to work (35 min from my home), parked my car and that is the last thing I recall for approx 2 hours. Had grand mal seizure 1 hour later that lasted 2-3 minutes. Also found to have UTI that had no symptoms of as well severe in nature.

Within first 12 hours after shot fever 104.8, chills severe body aches and lymph node symptoms , nausea extreme muscle aches, excessive urination . This all lasted 3-4 days. Now I?m on day 6 post vaccine . I have bad headaches, fever 100, lymph nodes swollen, pain in my back of my neck on the left side that radiates up into my head. Nothing is relieved by Advil or Tylenol. Benedryl does help a little . I have developed a rash on my left breast. Still have Pain swelling and redness at the injection site. And extremely fatigued. Going to the doctor tomorrow. If I had the chance I would NOT get this shot again and do NOT recommend it to others . I have been extremely ill since I have received this vaccine.

4/12/21 ER: Pt c/o palpitations onset 30 min pta. Pt states had second covid vac. At 0830 this am. Pt denies pain. dc home DX: Palpitations, Sinus tachycardia

The occurrence of additional non-serious events is detailed below. Concomitant products included LEVOFLOXACIN, RIVAROXABAN (XARELTO), LISINOPRIL, HYDROCHLOROTHIAZIDE, OXYCODONE and LEVOTHYROXINE SODIUM (SYNTHROID) for an unknown indication. On 12-Feb-2021, the patient received first dose of mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) 1 dosage form. On 12-Mar-2021, received second dose of mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) dosage was changed to 1 dosage form. On 29-Mar-2021, the patient experienced GAIT DISTURBANCE (hard time walking) and WALKING AID USER (had to use a walker). On 12-Apr-2021, the patient experienced THROMBOSIS (having a blood clot down whole right leg / up right side into right chest / under right breast / several clots in her lower abdomen) (seriousness criteria hospitalization prolonged, medically significant, life threatening and intervention required) and LOSS OF CONSCIOUSNESS (Kept losing consciousness/Passed out) (seriousness criterion medically significant). 12-Apr-2021, the patient experienced FEELING ABNORMAL (she almost died) (seriousness criteria hospitalization and medically significant). On 26-Apr-2021, the patient experienced PERIPHERAL SWELLING (right leg is very swollen). On an unknown date, the patient experienced URINARY TRACT INFECTION (urinary tract infection). The patient was hospitalized from 12-Apr-2021 to 19-Apr-2021 due to FEELING ABNORMAL and THROMBOSIS. On 12-Apr-2021, LOSS OF CONSCIOUSNESS (Kept losing consciousness/Passed out) had resolved. At the time of the report, THROMBOSIS (having a blood clot down whole right leg / up right side into right chest / under right breast / several clots in her lower abdomen) was resolving and FEELING ABNORMAL (she almost died), URINARY TRACT INFECTION (urinary tract infection), GAIT DISTURBANCE (hard time walking), PERIPHERAL SWELLING (right leg is very swollen) and WALKING AID USER (had to use a walker) outcome was unknown. DIAGNOSTIC RESULTS (normal ranges are provided in parenthesis if available): On 12-Apr-2021, Computerised tomogram: blood clots (abnormal) blood clot. Patient stated she was in the ICU24-48 hours then transferred to another area of the hospital where she spent another 5-6days. She is still having to use a walker. Her right leg is very swollen. She has to walk and keep her right leg elevated. Started off as sharp back pain on left side coming and going for several weeks, beginning on April 12, 2021. On May 4, 2021 she went to the emergency room and was diagnosed with kidney stones.

Presented to Hospital with HAs, confusion, septic shock, AKI. GTC on day of discharged, prompting transfer to hospital.

Patient described her symptoms as "layered". About 5-6 hours after vaccine symptoms began. Tight, hot pain developed in neck. Patient took Tylenol. In the morning of 04/21/2021 pain had moved to shoulder and by 04/22/2021 morning the pain had reached her lower back. 1pm on 04/22/2021 patient developed severe urination. Patient was urinating 2-3 times an hour for the next two days. 04/23/2021, Patient's Dr. thought it was kidney stones. Patient went to Hospital 04/23/2021 where they did a CAT scan and bloodwork. Patient was prescribed tramadol, lidocaine patches and physical therapy. Other symptoms included headache, palpitations, varying blood pressure, arm soreness, fatigue and blood in urine.

?Brainstem Encephalitis? Patient admitted to inpatient on 05/16/2021. Presented to the ED with dizziness, nausea, general fatigue and diplopia. Patient also voiced an intermittent mild headache. Patient was evaluated at an Urgent Care on 5/14/2021 with a ?presumed viral type illness?. In the ED, the patient denied neck pain, photophobia, eye pain, one sided weakness/numbness, facial droop, cough, congestion, and SOB. Temperature max at home was 100.8F. The patient does have gaze deficit and nystagmus on exam. Negative toxicology screen. Negative SARS-COV-2 Antigen. ALT 86. Medical history includes: hypertension, depression, Vitamin D deficiency. Neurology- This is a 56-year-old patient referred to Neurology secondary to diplopia. The patient states that her symptoms started with generalized fatigue and "not feeling well". After 2 to 3 days [last Thursday 5/13/2021] she began having difficulty with double vision. She has double vision in multiple directions. The double vision is fairly constant. Along with the double vision she feels off balance to the point where she has difficulty with walking. She does admit to internal vertigo. Patient denies any recent viral infections. She is about 4 weeks out from her last COVID-19 vaccination which was Moderna. During the hospitalization the patient has developed urinary retention requiring a foley catheter. Hospital medications include IV Solu-medrol and subcutaneous heparin. Patient seen by internal medicine, neurology, infectious disease, and urology during hospital stay. The patient was discharged on 22MAY2021 after demonstrating ability to ambulate. Will continue with physical therapy outpatient. See Tests and Lab Results Below

Developed flu like symptoms about 6 hours after receiving the vaccine. About 12 hours after receiving vaccine, I developed tachycardia. Saw primary care doctor the next day. About 2 days later, I developed pain in my right side. Bedford Memorial Hospital Dr. said it was a severe case of Shingles. A raised rash developed, but due to the antiviral medication the ER Dr gave me, it seemed to slow the rash and keep it from developing the fluid filled bumps. Where the rash is subsiding (after 2 weeks), I am still dealing with unbearable pain that wraps around the right side of my torso from my spine to my stomach. I was taking Morphine (Ibuprofen and Gabapentin) to control the pain, but because of severe gut issues, I stopped the Morphine & Ibuprofen. I'm currently taking the Gabapentin and Tylenol. However, my gut issues remain. I have no appetite and cramping and diarrhea . I'm heading back to the Dr tomorrow for more testing.

Loss of smell day after 2nd shot, maybe 70%-85% degraded from exceptionally acute sense of smell to nearly nil. I had underlying body inflammation prior to getting vaccine, but otherwise no illnesses or issues. Two days after vaccine I came down with a UTI. Another 3 days after that my body went into an acute inflammatory response. I had extremely pronounced pinkeye (first time ever) followed by acute gout flare-up (first time ever) followed by extreme edema in lower extremities (first time ever). I'm being treated with steroids, but these issues have yet to resolve. I have not been able to work since Jan 4. I can only stand for a few minutes and walk a short distance with the aid of crutches. In all of my life, I have never had pinkeye or gout or edema, or a UTI for that matter. I am a healthcare worker and I'm needed back at my job asap.

Two hours post vaccination: tongue heaviness followed by metallic taste sensation, vomited once after this. Four hours post vaccination: nausea, bilateral forearm pain evolving to all over body aches and muscle cramping along with a pulsating headache. Spiked a fever of 102 F that I treated with 500 mg of Tylenol. Later took ibuprofen for body aches. Had frequent urination along with unexplained crying. The lowest the fever would measure for the next 24 hours was 100 F. I was taking Tylenol and ibuprofen as directed on a regular basis. On Thursday 1/21/2021 at 5:55 pm, I woke up drenched in sweat. I felt weak yet improved. Headache and back pain were less. Showered and moved to the couch in living room attempting to watch TV. Could not stay awake. Woke up three hours later again drenched in sweat. Showered and went to bed. Slept through the night and woke up at 8 am with body aches, muscle cramping and nausea gone. Night gown and sheets were damp with sweat. Lingering symptoms: Skin from lips and inside my mouth appears to be sloughing off. There are very tiny cuts on my bottom lip. Left with mild headache I attribute to dehydration. No longer nauseous although I have no appetite.

Chills, fever, achy joints, high heart rate, UTI symptoms. UTI symptoms and achy joints happened same day as vaccine. Achy joints a couple hours after vaccine. UTI at 14:15, that hit me instantaneously. Chills, fever and high heart rate hit me the next day. 12/31/2020. That's when I went and the sent me to the ER.

Started having pain and difficulty urinating last night. Had to get up during the night to go a few times as well. Calling doctor?s office this morning. It is a bit concerning as my brother ended up with a UTI a few days after his second shot and was treated with antibiotics

Starting the evening following getting the vaccine, patient developed chills and upset stomach. The following day, she dealt with diarrhea, chills, and headache. On day 2, following the vaccine, patient felt overall well until dinner time, when she developed sudden urgency and urinary frequency. That same evening, patient developed mild lower abdominal pain that is worse with palpation. Since receiving the vaccine, patient has felt more fatigued than normal and this symptoms continues to persist.

Symptoms began the day after the injection was received. Symptoms included: high fever (103 degrees), nausea, lightneadedness (almost passed out twice), uncontrollable chills (whole bed was shaking), night sweats, headache, mental fogginess. The day after the injection I developed a UTI; was put on macrobid antibiotic. Shortness of breath began on day 3 after the injection. Difficulty breathing; out of breath going up the steps. Lip tingling; right ear ringing; numbness to face; head pain approximately 1 week after receiving vaccine. None noted prior. Patient thought she had an ear infection (with fullness to ear), and sought care for that vs. thinking possible vaccine symptom. Patient symptoms are ongoing 1 month post vaccine. Referred back to PCP.

Patient was a patient with long time diabetes and hypertension. She received the Moderna vaccine last Thursday. She had some nausea and vomiting and later some diarrhea. Unfortunately, she continued to take her diabetic meds and antihypertensives. Her CT abdomen showed duodenitis. She presented with multisystem organ failure and severe lactic acidosis. Despite aggressive treatment, she died less than 24 hours after admission. She was a complex patient and the renal failure and metformin toxicity seemed to be the terminal event, the vaccine may have started the process with some dehydration from nausea and vomiting.

Patient with left upper extremity pain and tenderness. Materna Covid vaccine given initially in April and then second dose was on May 22. After first dose she noted left upper back and neck pain in the early mornings which would resolve throughout the day. This continued until the second shot. After the second shot she had a fever and chills for 1 day. She was doing okay after the second shot up until a little over 2 weeks later last week on May 8 where she developed significant pain in her left upper extremity she went to Medical Center. At the time there was no redness apparently and it did not appear to be cellulitis. They felt it was a muscle strain and they gave her muscle relaxants and steroids. She had minimal if any improvement with that. The area started become red 3 days ago and therefore she presented here. She has not had fevers recently. Appetite has been poor since May 8 when pain started.

Admit note patient is a 57-year-old female with past medical history of diabetes mellitus II, hyperlipidemia, COPD and recent history of anoxic brain injury for which she was admitted at hospital and subsequently discharged to hospital for completion of rehab. Patient presents to hospital via EMS due to significant dyspnea observed by staff over the previous 24 hours. Per medical record tab, patient was having difficulty maintaining O2 sats and was evaluated by Dr, sats were 77%. Patient had labs ordered and BNP revealed value of 90,000+. Patient received dose of IV Lasix at facility for pulmonary edema and alternated between nasal cannula, CPAP, nonrebreather's and Venti mask. Patient ultimately achieved 85% sat on Ventimask subsequently transported to ED this morning. Of note, patient was functionally independent and lived with daughter prior to being found down and during late May and subsequently admitted to hospital where she was intubated, treated and subsequently discharged to facility for completion of rehab. Per facility staff, patient was able to speak and answer questions with baseline degree of expressive aphasia just 24 hours prior to presentation hospital. On evaluation at admission, patient appears alert but is not able to answer questions or obey commands. Thus, history was limited to information obtained from rehab and ED checkout. In the ED, an ABG was ambulated which revealed pH 7.35, PCO2 64, PO2 49, bicarb 37. CMP was significant for BG 135, BUN 27, creatinine 1.3, magnesium 1.5, electrolytes otherwise were largely within normal limits. Initial troponin was elevated at 87, BNP was significantly elevated at 111,902. Pro-Cal and lactic acid were negative within normal limits. CBC was largely within normal limits. Urinalysis demonstrated moderate blood with small leukocyte esterase. Vitamin D was significantly reduced to 15. Covid screen was negative. A CT PE was completed and negative for evidence of PE, however demonstrated bilateral pleural effusions body wall edema and ascites. Patient was subsequently admitted to Hospital for further management Neuro admit note: 57 year old female with DM, COPD, HTN, h/p anoxic brain injury. was at nursing home when started having desaturations. BNP was elevated and patient received oxygen and Lasix. Patient was unresponsive and had to be intubated. Was not moving left side very much stat head CTA was done which was negative, was hooked up to eeg and patient was found to be in status epilepticus. currently she is on keppra 1g q8h and propofol. She has had no clinical seizures today and is moving all 4 ext. MD. Working diagnosis Acute disseminated encephalomyelitis associated with COVID 19 Vaccine.. Patient responding to high dose steroids (1gm methylprednisolone daily) x 5 days. Extubated, and alert and oriented. Responding to commands. Only one oligoclonal band. Still in ICU, but preparing for transfer to lower level of care.

I have developed blood clots in my brain one from arterial dissection in my neck on the left and now one in another artery.

Starting immediately after the vaccine, I had flushing red and hot across my chest, numbness in my left hand and horrible nerve pain shot down to my fingers. I also had pain in my back and shoulder. After the 30 minute waiting period, I tried to get up to stand and my foot was asleep and had "firey" nerve pain. When I got home, my nerve pain worsen and I had whole body pain. I have horrible headaches, eye pain and extreme left arm pain at injection and all the way down to my elbow. It a very deep pain. The pain is above a 10 of 10 and not control with my regular pain meds (baseline pain before covid vaccine 6/10). It seems like the COVID vaccine has made my body pain unbearable and more intense. I also have been having to urinate every 30 minutes.

1st dose December 23, 2020. Arm pain, swelling, raised red 3 inch bump developed within 12 hours. Subsided, and reoccurred about 10 days post vaccine. 10-14 days post vaccine I developed New onset forefoot weakness (foot drop) Right, new onset trigger finger Right middle finger, new onset bladder spasms with immediacy and incontinence - not bladder volume dependent, resurgence of body aches, new bilateral knee pain requiring two arm support for sit to stand, Mental status changes including anxiety, paranoia, mania and depression (personality change!), motion induced nausea and dizziness, visual changes (blurry vision), New intensification of nocturnal muscles spasms - bilateral and progressive and intermittent headache up the Right neck to crown, New onset (01/14/21 - present) burning rubber scent when some food being cooked. The muscle and joint pain were present with February illness, subsided by April, and came back with a vengeance within two weeks of the 1st injection. I also lost three teeth (fracture vertically while flossing), thinned hair, and had intermittent dizziness during the initial illness in February. February Initial loss of taste/smell included metallic taste with artificial flavors, rotten garbage - random, and ?iron taste with meats. I can now taste some fruits but only get bitterness with other fruits and vegetables. I had one sided facial paralysis after tooth extractions in September - resolved in 1 week. I required dental implants to replace the fractured teeth. I am a vaccinator, and have been administering our vaccines and swabbing for Covid testing since we reopened elective procedures. I strongly encourage our staff to get the vaccines - and I am the one who gets the reaction. The constellation of symptoms, temporal proximity to the vaccine with no intervening illness or injury, and commonality of some symptoms with my February illness have prompted further testing and evaluation for central nervous system insult.

Dizziness, vertigo, nausea, weakness, hypotension, generalized body aches, dehydration, severe effects lasted 6 days, lesser effects 2 additional days. Right arm (injection arm but not site specific) entire arm extremely painful. No redness or swelling noted. Noted extreme amount by volume of urination for 2 days. The first 12 hours voided large amount every 1-2 hours.

Injection on 1/27-- On 1/28 chills, no temp, pain in left arm including injection site, underarm and left chest, extreme fatigue and though intake was limited excessive urination was noted. 2/11 heart noted to be above 120 at home to ER where heart rate was over 110.Shortness of air with increased heart rate. 2/15 Resting heart rate remained elevated and left side chest and arm pain.

Per Dr's admission H&P: " HISTORY OF PRESENT ILLNESS 58 yo F presenting via PH EMS with complaints for fever and SOB and weakness and dizziness. was brought in on 3L NC with Spo2 around 93 but was down to 84% on RA. initialy BG was 79, s/p 15g oral glucose with BG up to the 90s. speaking with patient she will awake to noise and starts her story and hen will stop. starts off by saying she had her covid shot on wednesday. patient denies any pain. speaking with her daughter over the phone, reports she had her COVID shot on wednesday and started to have HA and get weak and sob. reports she believes this is a reaction to the covid vaccination because the same thing happened to her when she received the singles shot a couple of months ago. reports she had a mild reaction when she received the first covid shot which was feeing tired and arm pain. 25 mins. after injection sore right gland, an hour and half later a slight headache and left shoulder/middle of back slight pain . By evening bilateral kidney pain back pain, gland still sore and some chills without fever. after midnight bad back discomfort, skin sensation and have had weird dreams since first vaccine and increased with the second which im still having as of last night By Tuesday-day two the pain and discomfort was all on my left side of chest and back from the side of my body to the center of my back -which is the side of my injection, skin was so sensitive to touch even my breast hurt and right gland pain, chills returned that night, Wednesday was the same that evening had the worse chills ever but no fever and then by Thursday most everything subsided except for the arm and shoulder pain from the injection which lasted 7 days

Products known to have been used by the patient included alprazolam, atorvastatin calcium, biotin, macrogol 3350, black cohosh, calcium carbonate/colecalciferol, minerals/vitamins NOS (not otherwise specified), celecoxib, eszopiclone, fluticasone, HCTZ, levothyroxine, pregabalin, montelukast, oxycodone, paroxetine, fluticasone proprionate/salmeterol xinafoate, potassium, risedronate sodium, and topiramate. On 22-Feb-2021, approximately four days prior to the onset of the symptoms, the patient received their first of two planned doses of mRNA-1273 (Batch number: 012A21A) intramuscularly in the left deltoid muscle for prophylaxis of COVID-19 infection. On 26-Feb-2021 the patient got a kidney stone and the doctor gave her medications. On 27-Feb-2021 she got a fever close to 101.4 degrees F for two days. Aspirin was taken which helped. A red mark was noticed on the injection site on 1-Mar-2021 which was described as an itchy lump. The patient also experienced coughing since receiving the injection. Treatment medications for the events included aspirin, ciprofloxacin and tamsulosin. The event kidney stone/nephrolithiasis was medically significant. The action taken with mRNA-1273 in response to the events was unknown. The outcome of the event, fever close to 101.4 degrees F, was considered recovered/resolved on 1-Mar-2021. Patient had nose bleeds, loss of urine control, severe leg cramps, nausea, fever 102, body aches, headache within 24 hours of receiving vaccine, she started to fill some better after 24 hours, treated symptoms at home

I received the Moderna shot #2 on Thursday 4/22 at 12pm. Had tenderness around the shot sight but nothing else. Started getting nauseous around midnight. Didn't sleep well. Friday morning abbout 3-3:30 am, I took one Tylenol. At about 4 am i was very nauseous and thought i better try to get to bathroom. I started to get out of bed and had both feet on floor when apparently i passed out and fell back into the bed. I never felt like i was going to pass out. I had no warning. I was just out! My husband said i then started to make weird sounds, like i was throwing up (but didn't throw up). I then went silent and he spent about 45 seconds to 1 minute shaking me and hollering my name continuously trying to get me to respond. He said he couldn?t detect a breathe. He said i finally took a huge loud deep breathe and came to. I don't recall any of the above. All i recall is hearing someone very faintly calling my name over and over but i could not respond. When i finally came to, i realized i had relieved myself in the bed and was very nauseous and threw up. After that had severe chills, and extreme fatigue all day. I ended up sleeping all day. Had nausea quite a bit of the day. Had light headaches towards the end of the day. Slept all Friday night. Got up Saturday and felt fine (about 98% back to normal). Since then have had soreness at injection site and tiredness. Just thought you might want to know my reaction to the 2nd shot. I am worried about taking a booster shot in the future if this was my reaction to the 2nd shot.

No urgency/alert with regard to full bladder urgency. Awakened by moderate pain & discomfort with no urge to urinate. After emptying very full bladder, pain subsided. No treatment was sought. Lingering morning lack of urgency. No pain. No treatment.

Pt began feeling ill the evening of her 2nd Moderna COVID vaccination 4/27 in which she had c/o fever, body chills, n/v in which she was unable to eat/drink, dizziness, generalized weakness and achiness. In the ED, she was found to be dehydrated with an AKI and hypotensive. Head CT showed a likely new evolving lacunar infarct in the right external capsule. Treated with 2L of fluids, low dose levophed and abx. Blood cultures were negative. Neurologist added aspirin to her regimen.

Chronic UTI. UTI symptoms started on 1/24/21. I have been on five courses of antibiotics; Keflex, Cipro, Bactrim, MacroBid and a second round of Cipro. Symptoms of infection are persisting today 5/26/21. I have had full pelvic ultrasound, been to physical therapy for pelvic floor exercises. I will visit a Urologist today for further treatment of this persistent infection.

Sudden onset of extreme vaginal pain, blood in urine. Tests/exams were done but nothing unusual was found. There was no treatment. Problem resolved after about 12 days.

Red, round itchy spots, bumps/rash on under arms, abdomen, lower abdomen, upper thighs, and lower legs. Very itchy, very inflamed. Lasted over two months, until treated with steroid injection, oral steroids, and topical steroids (on 6/25/21). A few areas left slight scarring. Also had some issues urinating. Had kidney scan on 6/22/21 and blood work on 6/7/21. Still investigating that issue. Apeear to have recoved fully from rash but treatment was completed recently (6/30/21).

Prior to the vaccine, I had never had symptoms of COVID nor ever tested positive for COVID, so I assume that I had no prior infection. I felt in the best of health, and my only health issues were weight (5?7 195 lbs.) and sciatic nerve and right hip pain that are not present if I walk and do yoga. I had no abnormal labs, or blood pressure issues or take any medications. I only take multivitamin (EmergenC Immune and Mega Krill Oil, and presservision (recently added). My routine activities were daily walk 2 to 3 miles, yoga 2 X per week, gardening, and horseback riding. Timeline of symptoms: I received my first dose of Moderna COVID Vaccine Feb 4th and my only adverse issue was a sore arm. I signed up for VSAFE and entered my symptoms as contacted. I received my second dose of the Moderna COVID Vaccine on Mar 4th at 1:30pm in my left arm with horrible onset of symptoms in less than 24hrs. I woke up Mar 5th at 4:00a with horrible random sharp shooting nerve pain and muscle pain worse than any pain I previously experienced with the flu or shingles, and I felt like I had been hit in the back of the head with a baseball bat. The ?head? pain was localized to where the spine attaches to the head. I have never had a headache of any kind in that location. I could not go to work nor get out of bed. I emailed my boss at 11:33a that the random sharp shooting pain seem to have subsided. By the next day all the lymph nodes in my body were swollen and symptoms progressed to where I had numbness from elbows to hands, and knees to feet and pain in my right arm (vaccine was administered in left arm) and neck when I moved a certain way and extreme fatigue and mild to moderate nausea. I started taking a magnesium supplement to see if that would help the pain. March 11th emailed boss that hands and legs were feeling more tingly than numb now and pain seemed less. March 15th email to boss no improvement, I thought I might have transverse myelitis since that is one thing I found when searching vaccine trials. The first neurologist appointment that I could find was March 24th. I reached out to primary care and she was able to see me March 17th. I described the fatigue as mild and not further improvement to numbness/tingling in hands, legs, feet, and pain in neck and back. The pain was uncomfortable but not unbearable. On March 17th, I went to first available appointment with primary care physician and she ordered MRI and lab tests and found no abnormalities. MRI showed lymph nodes swollen and arthritis in one hip. I was still able to do my daily walking, but household chores made me feel nauseous. On March 20th, the sensation of touch came back to my hands and on Sunday March 21st the sensation came back to my feet. I emailed boss on March 22nd they were mildly throbbing painful and achy and sometimes feel like bugs biting but I was thrilled that the numbness was gone. On March 24th emailed boss that I had to take the day off the aching and throbbing in my hands kept me from sleeping but still thought this was better than numbness. Handwritten notes for March 24 say aching throbbing veins March 25, Vein burst in left hand. This hand may or may not have had prior venous stick during MRI on March 17th, but hand looked fine no bruising or trauma. I was just standing and talking to coworker when it happened. I had weird dilation of veins in feet. I went to the ER and the doctor ordered a blood tests for bleeding disorder associated with Moderna vaccine. She told me to consult hematologist. She said to come back if I had petechiae or bleeding gums I took off my socks and shoes and asked her to look at the veins in my feet and she glanced at them and said they were fine. I was told all lab tests were normal. Found handwritten notes vein in hand blew, ER said all tests normal. On Sunday March 28th, I emailed boss that my veins now feel like they are burning inside, then feel tight and then get enlarged. I felt tired but no bleeding gums. Handwritten notes say burning veins in feet and hand, then feel tight and become raised On March 29, 2021 I took off work again and emailed coworker that my veins were being attack now. The veins in my feet had the sensation of burning like with IV meds. Later I had crushing sensation on left side of chest. I went back to ER and told about the veins and had them consult with neurologist and was told there was no treatment. I was told the X-ray, EKG, and labs were normal. Later found out when requesting medical records that BUN lab was high. I also recall complaining of difficulty breathing was treated like I was having some sort of ?vaccine hysteria? and doctor wanted to administer Ativan, but I declined. My handwritten notes say that the crushing sensations lasted until 4/1. My handwritten notes for 4/2 state blurry double vision when reading and mono-like symptoms On April 5th, I emailed boss that ?I?m still alive but have mono-like issues, excess sleeping and blurred vision and at base of skull. Nerve issues were minimal in legs, feet, and hands. I was still able to do my walking. My handwritten notes say fatigue. April 7th, my handwritten notes for 4/7 state moderate neck pain and throbbing at base of skull. April 8th My handwritten notes for 4/8 state moderate neck pain, throbbing at base of skull, left side of neck pain, lymph notes swollen, arm pain, veins in feet burning again. Driving home sudden stinging pain left front side of head, tingling in lips. Sting was a second, tingling lips a few minutes. April 9th My handwritten notes for 4/9 less neck pain, veins in feet just feel tightening but not burning, lymph nodes went down some, burning sensation in upper spine. April 10th Handwritten notes Intense base of skull and neck pain, tiny bumps on torso and arm. Note: these are not visible just by touch?not a red rash. April 12th handwritten notes Not a red rash on torso, burning sensation tiny bumps like when cold. April 13th handwritten notes lump in throat feeling, intense pain left foot arch, bottoms of feet burning no, only a few veins in feet tight and burn now, body itches when walking in heat 80 degrees. April 16th to 4/22 handwritten notes ankles, wrists, fingers, toes, foot arches, and heel increased pain. Ankles, heels, foot arch painful, both wrists painful, middle finger right hand painful, aching joints, fingers toes. Sensitivity to light, visual migraines without pain, extensive eye exam and optic nerve said OK. Inner ear discomfort. Burning sensation in abdomen. Friday April 16th emailed coworkers that it had been 6 weeks and I still feel I can barely get through a workday and the symptoms are so varied day to day but always include nausea and fatigue. Monday April 19th emailed boss that my vision has been distorted for several days-different than before that I need to see ophthalmologist. Ophthalmologist said eyes and optic nerve looked OK, but I explained I can?t read clearly, and they feel ?grainy? inside. I was told not treatable again. Currently, I recall telling the ophthalmologist that the pain was horrible. April 23 hot flashes, ear discomfort, lymph nodes still swollen Monday April 26th emailed boss that I felt like crap, I had issues with inner ears and vertigo but could still hear the same volume number. Still have fatigue and swollen lymph nodes-some other weird issues seemed better. Monday May 3rd, appointment with neurologist again. I went over all previous symptoms and mention I had developed plantar fasciitis and had to stop my daily walks. I was prescribed gabapentin and he mentioned on patient had taken Medrol. He had 2 when I first saw him and now had 3. Thursday May 6th emailed boss that I had to take off work due to extreme nausea after taking Gabapentin. I only took one day before bedtime but within 3 days, I had severe nausea, crying for no reason, and couldn?t write with a pen in right arm, urinary urgency, and weird dreams for a week. May 17th emailed for another medical appointment May 19th Reached out to neurologist through portal and asked if I could take the corticosteroids like other patient or had too much time passed. I was prescribed 4mg Methylprednisolone 6-day blister pack. I had significant improvement (80% reduction in symptoms after 3 days). May 27th, I saw primary care doctor as she had availability when neurologist did not. I still had I still had lingering issues of pain, swollen lymph nodes, and heart palpitations. I started a second round of 4mg Methylprednisolone 6-day blister pack and symptoms even more improved. May 27th Second eye evaluation said dry eyes and to treat with the eye drops that I had already tried and didn?t help much May 27th Massage therapy says right foot contraction during massage is odd. May 31st, ER visit-odd contractions of middle finger tendon on right hand, crushing tingling sensations on top of head but scalp doesn?t hurt, muscle weakness in both legs. ER ran thyroid and some other tests and said normal, said I wasn?t having stroke or any medical emergency and was sent home. May 31st Calendar says chiropractor. June 7th, Appointment with neurologist again. my pain is nearly gone but I do still have an occasional ?burning? sensation in left arm at the injection site and torso. The veins in my feet, calves, and head still have strange expanding/constricting sensations. Sometimes have tightening of the tendons/toe issues on right foot. (Note, every time I have medical massage therapy, therapist comments on how odd this foot contraction is in the right foot.) I have greatly improved mental clarity, but still significant deficits with ?working memory?. I still have some issues with my ears. I still can?t do any significant physical activity without the room spinning. I must be slow and steady in my movements, but I do try to do yoga in a chair and exercises like leg and arm lifts and twists in bed. I started taking PresserVision AERDS2 for my eyes but found out it significantly helped to calm down the cranial nerves discomfort. I only take in the evening before bedtime as it increases the nausea if I take it in the morning. June 14th, Appointment with ENT to evaluated difficulty swallowing and hearing issues. Left ear has some mild hearing loss, more tests ordered for later. June 16th Appointment to evaluate veins. Tightening feeling in veins in feet, still unusual dilation, aching calves, knotty veins in hands. June 21st Still issues with vertigo, ears, lymph nodes under chin, burning sensation down spine, torso, arms, legs, veins in feet, extreme fatigue. Spider veins in legs get worse on left leg. Annoying twitching in my left eye all day. June 24th Massage therapy. She says reflex in right foot is odd. June 25th Ultrasound on veins. The annoying left eye twitching seems to have stopped. June 28th Swallowing test. June 29th Appointment with rheumatologist. Lots of labs ordered. She said to start taking 500 mg Tumeric 2X a day which I did. July 2nd VNG testing done ordered by doctor. July 3rd Medical massage. She says reflex in right foot is odd. July 5th Started taking youth theory collagen 1,2,3 and noticed less burning, tightening, issues in legs, feet, and veins. July 8th Modified barium swallow test. July 12th Appointment with ENT. Issues with ears appear to be with Eustachian tubes. Some acid reflux issues with taking Tumeric now. He said to take Prevacid. He said to relieve the ?full ear? feeling by taking a deep breath, pinching your nostrils closed, and ?blowing? with your mouth shut throughout the day. Said to try FLONASE pointing towards the check to relieve pressure. July 13th Go to drug store and get Prevacid for the acid reflux and Flonase to see if it will open the eustachian tubes. Within a few hours, I developed a pounding heartbeat and headache and made it home and went straight to bed. July 14th Appointment with Rheumatology. I still had pounding heartbeat and headache and could tell something wasn?t right with my blood pressure. My blood pressure was 137/97 which I think is the highest I?ve ever had. Doctor said the results for the autoimmune showed no evidence of autoimmune disease. There is some lab that give more fine-tuned results, and she wanted the labs repeated and sent to there. I made it through my workday and still had headache and pounding heart. July 15th, I feel OK today-the usual nausea and fatigue and occasion burning sensation with no headache or pounding heart. The one dose of Prevacid 24hr on July 13th is all I will be taking. In the past, when I had prior symptoms of acid reflux, I took this medication without incident. July 15th Appointment with gynecologist. July 16th Appointment with Immunologist. Summary As time went on, I developed the sensation of bugs biting but thought I was getting better because the sensation returned to my hands and feet. These symptoms have progressively worsened until I feel the worse than I have ever felt in my life with fatigue significantly worse that what I have experience in the past with mono or pregnancy. The neurological pain was significantly worse than I experienced with shingles, and the nausea hasn?t gotten better. Since initial onset, I developed burning in my veins, blurred vision when reading and eye discomfort, followed by inner ear hypersensitivity issues, painful plantar fasciitis plantar and a burning sensation in trunk arms legs, night sweats, hot flashes, heart palpitations, crushing left side of chest, tightness feeling in lungs, tingling lips, difficulty swallowing. The neurological pain has varied from a ?bug bite, to fire ant, to bee sting? throughout my head and body to almost unbearable by May, development of spider veins on my legs and difficulty swallowing and extreme dizziness-the room is spinning. I have not been able to work a full pay period since the second vaccine and I am so fatigued that I spend the remainder of my time in bed sleeping or watching TV. _____ For months I was told there was no treatment and had several trips the Emergency Room with my disturbing vein in hand burst and vein burning and left side crushing symptoms and difficulty breathing sensation. May 3rd returned to neurologist and was prescribed Gabapentin at night. The first day is seemed to reduce the pain some, but by the 3rd day the side effects became horrible nausea, crying for no reason, and inability to hold pen in right hand, urgent urination. I had weird dreams for a week. At this appointment, the neurologist mentioned that he had another patient that responded to corticosteroids, so I reached back out and asked if I could try it and that is what I was prescribed. I had significant improvement (80% reduction in symptoms after 3 days) after starting 4mg Methylprednisolone 6-day blister pack on May 19th, but I still had some symptoms and my lymph nodes are swollen and many questions as to ?what next?. My eyes are much improved with the oral medication. I reached out to see if eye drops would further benefit my eyes but can?t get a portal or call or response from a doctor. I didn?t have a cataract at my last eye exam, and I didn?t notice the cataract until after vaccination and these issues started. At my recent exam, the eye doctor said it was due to age, but my eyes have felt grainy and blurred and vision distorted when I went there, and now I have some improvement with oral medication. My ears were hypersensitive to noise, but after steroid treatment felt more like when on an airplane, then eustachian tubes became painful and then closer to normal. My brain, eyes, ears, and veins had a lot of strange sensations during this first pack. I visited my primary care doctor on May 27th as I still had lingering issues of pain, swollen lymph nodes, and heart palpitations. I started a second round of 4mg Methylprednisolone 6-day blister pack. I felt continued sense of wellbeing and not toxic. The pain continued to decrease and is barely noticeable by the end of the second pack. However, on May 30th, my lymph nodes finally went down, and I experience weakness in my legs, the contractions in the tendon in my right hand and tightening sensation of the veins and nerves across the top of my head, then one or two days later, I experience them weakness in the right arm with the brachial neuritis. The weird sensations in my brain are quite disturbing, but hopefully it means it is returning to normal. The plantar fasciitis pain was resolved and though somewhat ?wobbly? I was able to return to daily walks and do yoga ?tree pose? and in a week the strength is back. The arm is still weak to bring a heavy mug or cup to my mouth, but I haven?t done any type of strength building exercise. Today June 7th, my pain is nearly gone but I do still have an occasional ?burning? sensation in left arm at the injection site and torso. The veins in my feet, calves, and head still have strange expanding/constricting sensations. Sometimes have tightening of the tendons/toe issues on right foot. I still feel tightness in neck lymph nodes. I still have current symptoms cranial crushing tightening sensations like an octopus is on the top of my head-but not painful. Can feel tightening in front of face and teeth and back of head. Neck muscles feel tight but don?t hurt. Issues with eyes-pain and discomfort most in right with only minimal short-term relief with ?Refresh? eye drops. Burning sensations in lower legs and mostly left arm where injected more then right arm, some burning sensation in torso, occasion biting sensation on extremities, Eustachian tubes still have discomfort that gets worse if I do any faster walking. Still can?t do strenuous activities like gardening or yoga without getting dizzy and feeling like I can?t breathe. I used to find walking invigorating and now it is exhausting. I still must be very careful when swallowing. The immunological symptoms seem much worse evening and night than during the day. The addition of youth theory collagen I, II, III supplement seems to have significantly reduced burning sensations. I still have periods of numbness come and go in extremities. I still have issues with tendon/toes of right foot. July 13th evening to July 14th?No 24 hr Prevacid-whatever neurological changes have occurred since March 4th, my system no longer tolerates this medication. Pounding heart, headache and BP 137/97 the next day. July 15th had appointment with gyn, no issues found, my pounding heart, headache issues from Prevacid resolved and bp was 120/82. She suggested that I seek therapy about my longterm health issues after the 2nd dose of Moderna vaccine, and suggested to see if the long haul COVID clinics also take pts with adverse vaccine reactions. The rheumatologist had suggested this as well. To my knowledge I have never had COVID. It was quite discouraging as she told me to do what activities that I could and that I may never go back to the activity/health level that I had pre-vaccine. Someone spoke with neurologist suggested that I have the N protein antibody test to see if I have COVID antibodies but the neurologist, rheumatologist and gynocologist didn?t know what that was or how to order it. She said stop taking the Tumeric and not have the acid reflux, rheumatologist said take the Tumeric to see if helps with antibody response. I still feel in my neck and throat that I have some immunologically going on that needs to be ?turned off?. I have visible damage to my veins and they still burn and ?tighten?, especially in the evening and early morning hours. She mentioned drugs to stop the nerves, but I want some thing to stop the antibodies that are aggravating the veins and nerves.

Patient received Covid 19 injection on 1/5/2021. On 1/6/2021 she had frequent urination, was diagnosed with UTI. Later at 6 - 7 pm she was having lighte headed, sweaty. Went to ER / diagnosed with New Onset Afib.

Bilateral Kidney pain started 49 hours after the injection. Kidney pain increased to severe level (7.5 out of 10) and patient was admitted to the ER 61 hours after the injection. Tests indicate that there was no blood in the urine which ruled out the likelyhood of kidney stones. tests also indicated there was no evidence of kidney infection. Tests did indicate dehydration. Two tylenol and IV fluids were administered. The kidney pain did not increase. Patient was released. The pain subsided over the next three days.

I forgot to take my Allegra and woke up with a big red rash on my left arm in the area I received the shot. My left arm was sore also. I took my Allegra the following day and the rash seemed to lesson. It started up again on 3/10/21 even though I have been taking my Allegra daily. I have had to urinate often since the rash. The clinic checked my urine and said it was clear.

Severe headache starting 10 hours after the vaccine associated with nausea, vomiting, incontinence. Denied LOC/seizure like activity. Was monitored during the entire time by her husband.

On 16 Mar 2021, the patient received their first of two planned doses of mRNA-1273 (Batch number: 027A21A) intramuscularly in the left arm for prophylaxis of COVID-19 infection. On 16 Mar 2021, two hours after the vaccination, the patient experienced lower body pains, had difficulty urinating, was not able to walk without a cane and could not bend over. She felt the vaccine may have aggravated the myelitis that she had in 2009. Treatment information was not provided. The occurrence of additional non-serious events is detailed below. The patient's past medical history included No adverse event. On 24-Mar-2021, the patient received first dose of mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) 1 dosage form. On 24-Mar-2021, the patient experienced NEPHROLITHIASIS (has developed a kidney stone) (seriousness criterion medically significant), INJECTION SITE INDURATION (hardness around the injection site), INJECTION SITE ERYTHEMA (redness around the injection site) and FATIGUE (fatigue). At the time of the report, NEPHROLITHIASIS (has developed a kidney stone) had not resolved and INJECTION SITE INDURATION (hardness around the injection site), INJECTION SITE ERYTHEMA (redness around the injection site) and FATIGUE (fatigue) outcome was unknown. The action taken with mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) was unknown. Patient called at approx. 1:00 PM and reported she thought she was having an adverse reaction to the Moderna vaccination. She complained of weakness, can't stand very easily, frequent urination, fever of approx 100 degrees. Pt actually stated she "fell out of bed" this morning. Blurred vision also reported

4/20/21: patient arrived to ER per EMS status post PEA arrest. Per ER records, patient became unresponsive while sitting in bed witnessed by husband at home. According to husband, they had come home, she sat on the bed and complained she was not feeling good. She then fell back on the bed and began to seize. Subsequently she had intermittent episodes of alertness and was able to speak to the husband followed by unresponsiveness. At time of EMS arrival pt. was unresponsive. EMS noted BS 120s, SBP 50s. En route to hospital, pt. had a CP arrest for which epinephrine was given, CPR initiated with ROSC. Pt. arrived to the ER on a NRB mask attempting to speak. Subsequently, pt. had several CP arrests with asystole, and

At 5:00 am I walked from bedroom to bathroom. I was unstable, legs felt weak, and needed to crab furniture, door to keep from falling over. The symptoms have not abated. At 8:00 am That same day I felt numbness in my feet, both of them. My husband drove me to the hospital approximately 25 minutes later. I checked into the ER. Since this time the symptoms I have included severe restless legs at all times of the day, continued inability to walk normally - instability, weakness in legs in particular the left leg. In addition, while still being checked into the hospital, 1 day later I have now developed numbness throughout my abdomen and lower back. I am not emptying my bladder, even though I'm using the restroom regularly. I am not able to feel urges to use the restroom either to urinate or defecate. It is as if the entire area is numb. At this time, I am still in the hospital and I am not able to walk without assistance, walker, people holding me.

pain and swelling began ~ 1 hour after dose at injection site, enlarged by evening to rest of upper arm, arm pit, shoulder and neck, still exists as of 5/13 at 2:30 pm although subsided some Fever and chills started 5/10, ~ 9 pm, ~10 hours after dose, lasting ~2 days. Next day 5/11, headache, constipation, dark urine, lasting ~2 days. Congestion and mild cough started ~2 days after. Treatment consisted of advil, benadryl, and sudafed.

5 days after first dose I had my first ever UTI. 4 days after second dose I experienced severe dry cough with some difficulty breathing, a fever ranging from 99.5-102 which lasted for 6 days, increased joint and muscle pain, severe nausea lasting 10 days with weight loss of 7 pounds. 4 visits to Urgent Care and one ER visit

Patient described that approximately 4 hours after receiving second dose, she developed headache, body aches, and chills. She stated that she experienced those symptoms until 2 days after when she developed a fever. She went to Emergency Room where her temperature read 103.9. She was subsequently admitted and diagnosed with decreased kidney function, pyelonephritis, and dehydration. She was discharged 4 days later. This information is all second-hand information received from the patient.

She got her vaccine, she woke up at 3:00 AM the next morning with a fever of 101, very very lethargic, heart palpitations, chills. She was so tired that she slept on and off for hours in a very deep sleep. She was not waking up enough to even make it to the bathroom. She stayed in bed for 2 days, and started to feel better that evening by 5:00 PM and slept for 2 days basically. For 2 weeks afterwards she was very fatigued and then started feeling normal after that and has been fine since that time. She did have some swelling and redness at the vaccination site with some pain. She called Teladoc and was advised to take Tylenol or Advil and she did which relieved the fever and felt better after that. She said with the first vaccine it felt like somebody had hit her in the arm with a board and hard to lift the arm. She works in a medical facility and a lot of them got really sick including the NP and the providers themselves and missed several days of work due to the reactions. They are also younger than her.

I just wasn't feeling well. I thought maybe the blood pressure meds I had been put on 6 wks prior were not working right, so I went to the doctor. They did bloodwork. My red blood cells are not making mature ones. higher than normal blood glucose, fever of 103.1 for 12 hours , nausea, diarrhea, muscle and joint pain, extremely cold under multiple blankets, headache , light headed, near syncope, extreme fatigue, long lasting brain fog((until approx June 1), racing heart, shortness of breath, frequent urination, weakness, loss of appetite. Ears rang louder vision blurry, my mild arrhythmia became more frequent/longer symptoms. I had to take 1 week off of work, and when I returned I still could not function normal 7 hour day. I had hard time staying up to normal bed time (11:00 p.m.), fatigue was chronic from waking to bed. Blood sugar dropped far below my normal lows after week one and have remained lower than normal lows)

A fter the vaccine, I initially felt fine. During a bike ride I began to feel weak, tired and my heart was racing. When I got home I tried to take my blood pressure and the machine said error. I was very tired and weak. Before I went bed feeling quite nauseous and felt overall weak and felt ill. Blood pressure was 85/65. During the night I was up often trying to vomit. I had some slight vomiting but mostly dry heaves. I was laying on the bathroom floor. My husband woke up and took me to the hospital. I was admitted for severe dehydration to the point it affected my kidneys. I had IV fluids throughout the day and was able to go home late that afternoon. The weakness continued for about another day and a half. Still a bit off but better as of 7/16/21

Hip and legs bone, muscle, muscle insertion extreme pain, increased heart rate, increased blood pressure, one episode of leg weakness, bright red lupus looking (malar) rash on face. It was hot to touch and burning. ER gave me toradol, steroids, and fluids. Facial rash and pain resolved that night. Bright red malar rash came back Saturday, Sunday, Monday.....comes back now when hot or out in the sun just on face.
