Patient complain of slight headacke, dizziness, O2 sat 97, Pulse 76, Resp. 24, was assessed by nurse for10minutes, patient begins to cough with wheezing noted, states " I am feeling my throat funny, no respiratory distress noted, continued to be monitored for another 10 minutes, no changes noted, states, I am feeling a bit tight in my chest, O2 sat at 96-97%, BP 124/58 was taken to ER via EMS for evaluation at 14:35p

After getting the Vaccine he fell down when he was in the store, with his father and so the father brought him back to the pharmacy and he had hit his back of head during the fall. And because of this he was little pale in color and he had no local reaction to the vaccine on his arm and he was communicating with us. we did call the ems services and they took him to the ambulance and per father he was let go after some time in the ambulance once he felt better.

Patient received their first Pfizer Covid vaccine and while his sibling was receiving her vaccine approximately two minutes later the patient fainted and fell on the floor and hit his head.

PT dizzy immediately following shot. Checked out by staff. BP low. PT hypotensive positive orthostatics. Medic administrated fluids (0.9% Sodium Chloride Injection USP 1000 mL). Transfer of care to another group. Transported to Medical Center.

Patient hands felt tingly, head felt tingly, got light headedness. patient was put into a wheelchair and taken to observation room by nurse. Patient was alert and oriented. Patient feels better after drinking some water.

Patient received the vaccine. Patient got very dizzy shortly thereafter. He started vomiting and slumped his head over in the trash can. BP was 98/58 with pulse of 56. Oxygen was 99. We got him to a hospital bed. 5 minutes later his bp was 102/60 with HR of 68. Pulse was now 79 and oxygen 99. 10 minutes out, his bp was 101/65 with a pulse of 72.

N/V, dizziness, taken via wheelchari to isolation area. Laying BP 103/52, HR 76. Sitting BP 94/74; HR 69; Standing BP 105/81; HR 86. Given water & juice w/no emesis. Pt had no breakfast prior & felt sleepy. After 10 min rest on guerney pt fel better. D/C'd in stable condition. Recommended to f/u w/PCP

Patient was administered Pfizer COVID-19 vaccine in store. Roughly five minutes later, he felt slightly lightheaded and collapsed in an aisle of the store. Pharmacist attended to the patient. He did not recall falling. He was observed to have a bruised nose and a small laceration on the lip. Patient's pulse and blood pressure were regular. Store manager provided water and an icepack. Patient continued to be monitored in store for an additional 20 minutes, after which time he stated he felt fine, and left with his mother. Patient had no history of reaction to immunizations.

Patient felt better after given juice and sitting down. 4:09 PM Vital Signs: BP 110/80 HR 107 O2Sat 99% Notes: Patient heart rate was still over 100 bpm. Other vitals were in good range. Cheeks still rosy with potential rash. Follow up with provider recommended. Parent provided CDC link with information regarding hx of GBS and safety of receiving COVID19 vaccine. Patient discharged home with parent.

The patient received his first dose of the COVID vaccine. The father states as they were walking out, the pt complained or "not feeling well". He describes this as feeling of dizziness and nauseaous. The father asked is he could come back to an urgent care room to lay down which is where he had a n episode of emesis.

Pt had vasovagal reaction and emesis immediately post vaccine admin. NO ALLERGY to vaccine. Okay to receive 2nd dose. At 1:37PM, 6 min post receiving vaccine, patient told mom that he felt lightheaded and was having pain at injection site left arm. Rn assessed immediately and pt reported feeling "sick". He drank few sips of water. MD called to bedside. I assessed pt at 1:39PM, pt sitting chair, elevated his legs onto the footrests of wheelchair sitting in front of pt. +Facial and lip pallor, and complaining of nausea followed by 1 episode NBNB emesis consisting of what he ate for lunch 1 hour prior. Pt immediately reported feeling better post emesis. Lips appeared pink. Pt brought to triage area, lay supine on stretcher with legs elevated. Nausea and "lightheaded" sensation resolved prior to lying down. BP improved post emesis from 75/39 to 84/46 at 1:48 PM. Stayed lying down for 5 minutes after which time pt wanted to sit up. Upon sitting BP and HR increased 96/64 and 69, stable with no nausea or lightheadedness, only remaining sx "slightly weak". drank 12 oz apple juice and ate 2 graham crackers without any further emesis or symptoms. Observed for 15 more minutes and was ambulating without return of symptoms, discharged home with mom and brother with instructions to seek medical care if symptoms return.

Syncopal episode - fell and hit his head on door No loss of consciousness Patient was possibly dehydrated or had low blood sugar, states that he had not eaten since 11:00am and had been playing golf all day until he arrived at clinic at 5:30 pm. Patient's father was present and took him home after evaluation by medical staff and sign off by medical director.

12 yo male presenting with bilateral lower extremity paresthesias and confusion. Admitted to the hospital. Pending discharge to rehab facility. ED Note 5/19/21 Patient is a 12-year-old male with hypogammaglobulinemia, asthma, IBS who presents with weakness in his lower extremities. 6 days ago, patient had seafood and later that night vomited once, nonbloody, nonbilious. 4 days ago, he received the first dose of the Pfizer SARS-CoV2 vaccination. That day, he also felt some burning over the anterior parts of his bilateral ankles. Over the past 3 days, he has been sleeping a lot and had a headache. He has not had a fever or other muscle aches, no subsequent N/V, abdominal pain. Last night, he reported ankle pain to his mom. Today at school, he was playing kickball when he had the sensation that his ankles were not working/gave out and he fell to the ground. He had difficulty getting up and required a wheelchair. He did not lose consciousness, denied CP, palpitations, SOB, headache, vision change during or before that episode. He was not incontinent of urine/stool and had no abnormal movements noted at the time. Since then he has noted twitching in his thighs, calves and toes. He reports weakness throughout both lower extremities and has difficulty with intentional movements at the toes/ankles/knees, weakness in hips. He reports a burning sensation over the anterior ankles/dorsum of foot b/l. Denies fevers, chills, cough, SOB, CP, palpitations, abdominal pain, N/V/C. He endorses diarrhea 3x daily which is his baseline. He has panic attacks 3x daily w palpitations/SOB that self resolve. He has been on keflex for an ingrown toe nail for the last week. MRI performed with normal brain and spine. However, on repeat exam patient has diminished sensation in the bilateral feet and diminished ability to dorsiflex or plantarflex both ankles with very limited toe range of motion. Still with preserved reflexes in the patella and Achilles. Discussed at length with neurology and the family. Differential diagnosis at this point of functional neurological disorder versus Guillaine Barré syndrome. Neurology note 5/21/21 Patient is a 12yo M with hypogammaglobinemia, IBS, and poorly controlled anxiety, presenting with 4 days of lower extremity paresthesias and 1 day of hyperacute onset of lower extremity weakness and perhaps some mild confusion. History notable for covid vaccine three days prior to presentation and anixety surrounding covid and return to school. His general examination is notable for anxious affect with difficult to localize neurologic exam. Exam shows possible bilateral lower extremity weakness of TA and hamstring>quad/IP that is very challenging to grade due to poor effort and giveway, and decreased sensation in a bilateral stocking/glove distribution to all sensory modalities. MRI w/wo contrast brain and spinal cord normal on admission. LP perfromed in ED also normal. Given negative workup and history of anxiety with recent psychosocial stressors, most likely sudden onset weakness is secondary to a functional neurologic disorder. Plan for discharge to rehab pending PT evaluation.

Patient received IM vaccine, was under observation when felt dizzy and lost consciousness. Spontaneously awoke a few minutes later and was confused, and wasn't able to feel his legs. Confusion resolved after about 5 min but leg numbness/weakness remained. Was taken via EMS to the hospital, with clinical assessment showing no neurological deficit, withdrawing to pain, and patient demonstrated ability to ambulate independently about 12 hours after symptom onset. CBC, CMP, Urinalysis, UDS unremarkable.

client received vaccine, ambulated to obs room, 3-4 minutes noted to slump down in chair, caught by paramedic-transferred to strtcher, pale, diaphoretic, BP78/40, HR84, o2 sat 100.

Pt recieved vaccination and at post vaccination waiting station, pt felt nauseated and a little dizzy. I raised patient's feet, gave him some water and placed a cold ice pack on his neck. He then threw up multiple times. After about 15 minutes, pt felt a lot better and left with mom

PATIENT FELT DIZZY, WE ELEVATED FEET FOR 5 MIN. PATIENT CONTINUED TO HYDRATE. PATIENT STAYED FOR AN ADDITIONAL 10-15 MIN. FELT COMPLETELY BACK TO NORMAL AND WENT HOME.

Patient received the vaccination at 1148. A few minutes after receiving the vaccine the patient became faint. The patient was placed in Trendelenburg, drank juice and remained laying for 20 minutes. VSS at 1103 when patient sat up. Patient did experience dizziness or blurred vision upon sitting upright. After sitting for several minutes his blood pressure was 100/59.

GAVE PATIENT OJ, WATER, GRANOLA BAR, TRIED TO HAVE HIM SIT IN CHAIR AFTER ABOUR 5-10 MIN, BUT PATIENT STATED FEELING LIKE GOING TO PASS OUT, HAD PATIENT LAY ON FLOOR WITH FEET UP ON CHAIR. APPROX 4:30PM PATIENT VITALS: BP 72/48, HR 81, O2 SAT 99%. PATIENT FEELING DIZZY, LAID ON FLOOR WITH FEET UP ON CHAIR, CONTINUED DRINKING WATER AND OJ. APPROX. 5PM PATIENT SITTING IN CHAIR, VITALS: BP 98/62, HR 95, O2 SAT 98%. PHARMACIST CONSULTED PEDIATRICIAN WHO ADVISED IF PATIENT FEELING OKAY AND NOT DIZZY/FEELING FAINT, OKAY TO LET GO HOME BUT TO REST TONIGHT AND TAKE IT EASY. ADVISED MOM TO TAKE TO ER IF ANY FURTHER FAINTING INTO THE EVENING. APPROX. 5:10PM PATIENT LEFT PHARMACY WITH MOM AND STATED FEELING FINE. MOM ACKNOWLEDGED SHE UNDERSTOOD PLAN FOR EVENING AND TO FOLLOW UP WITH PEDIATRICIAN REGARDING GUIDELINES/HOW TO HANDLD 2ND DOSE OF VACCINE.

Pt c/o nausea and dizziness. BP 104/37, HR 53, RR 14. Pt stable and released from the vaccination site.

Pt reported dizziness when getting up after his 15 min wait post vaccination. Only ate protein bar for breakfast & chips at noon. Mother reports pt has gotten dizzy after blood draws and gets really hungry around this time when his ritalin wears off. After water, juice & crackers pt reports dizziness is almost gone and he's ready to leave

AE: patient felt nauseous 15 minutes post vaccine administration and vomited, felt faint but did not pass out. Treatment: no medication was given. patient was under observation. Water improved symptoms Outcome: symptoms resolved and patient was sent to POV/Home.

During patients 15 minute wait he had a syncopal episode and fell out of his chair. Once I got to the patient he had come to, mom said he was out for about 10-15 seconds max and patient says he did hit his head. Oxygen saturation was 99% on RA, heart rate was 62bpm. BP 110/78. Patient was alert and oriented once I was assessing him. He was able to sit up in the floor about 5 minutes after coming to. He was able to drink water and the PCT had placed cool wet paper towels on his head and neck area. We provided the patient crackers and the management team was available. The patient said he felt like his stomach was upset and did not want the crackers. I suggested he go to the ER and mom said she would drive him and asked if this was caused by the vaccine itself. I explained what could have occurred and educated them on where he hit his head he needs to go to the ER to be assessed. Mom agreed and the manager got a wheelchair from the front of the store and mom drove him to the ER. I called report to ER.

After taking the vaccine patient stated that he felt like he was going to pass out. Mother stated that this happens every time he gets a shot. Patient was walked to chair where he was given water. Patient passed out briefly and was taken to medic station.

The next morning he woke up with arm hurting him (he couldn't lift it). He felt a little warm - had a low grade fever -99.something. He had a headache all day. He was very sluggish, tired. He did online school that day. He took a nap and laid down on the couch in between classes. He was really tired. I was giving him Tylenol every four hours. The first time he felt a little better, The second time, it didn't work. I called the doctor for advice and he told me to give him Children's Motrin. That seemed to do better. I gave that to him once that day. The next morning, he felt better. I gave him a Motrin anyway because he had a light headache. He did say he felt some heart racing. And he was a little tired. He called me from school around 11:30 am and he said that his heart has been racing through the whole morning - he was gym and said he couldn't run or anything but his heart was racing. I told him to go to t he school nurse - he had a pulse of 120. I met him there and I called the pediatrician and he saw him at 03:00. His pulse was high but not as high as before. He had rested in between. My son did see a cardiologist in June 2020- they put him on a heart monitor and she couldn't find anything at that time. At the regular doctor on the 19th, the doctor checked breathing and heart but did not have the materials at that location to do an EKG but said with the lowered heart rate just to watch and if it goes up again to go to Cardiologist we saw in June 2020. He rested some more and it went away on it's own. After that he was fine and no other issues. In fact, the next day he felt better and was running around and had energy and felt great.

Pt w/ hx of allergy to Amoxicillin (anaphylaxis, lip/face swelling). Pt c/o dizziness at 1410. Pt had a syncopal episode for a few seconds and woke up when reclined. Patient is A/O x4. Patient is cooperative, and denies pain or SOB. VS taken at triage, BP 86/54, HR 60, O2 97, RR 20, Temp 97.3. CN, nurse, and onsite provider were notified. Pt reports sweating, but denies tremor, numbness, tingling, nausea or vomiting. Offered juice and cracker to pt, no new medication ordered. VS repeated at 1420, BP 111/93, HR 85, O2 97, RR 16. pt reports improvement and denies dizziness, pt left triage with steady gait with family at 1430 and have further observation. Pt left clinic with family. Pt was advised to seek for emergency service if s/s worsen. All needs met, safety maintained.

Dizzy,nauseated Brought via wheelchair to medical screening for evaluation by medics. No interventions Left in stable condition at 13:03 p.m. Recorded by RN

Patient became light headed and lost consciousness for a few seconds. He felt better, but fainted again a few minutes later.

Possible Vasovagal reaction. Patient was approximatly 7-10 minutes post dose. Upon video review of event patient was sitting and stood, waivered (appearing dizzy), lost consciousness and collapsed to ground. Remained unconscious for under 30 seconds. Observed various muscle flexing/tension & uneven breathing. However patient never appeared to stop breathing. Patient did hit his head during fall & and was seen by EMS. During follow up patient was being seen in ED, but according to parents he appeared to be perfectly fine.

Patient reported feeling nauseous and vomited a few minutes after receiving his second dose of pfizer. Pt looked pale and reported feeling lightheaded. Pt did not show any signs of respiratory distress: no swelling of the throat, face or mouth observed. Vital signs were within limit: BP 125/72, HR 60, RR 12 (at 3:45pm), BP 120/74, HR 62, RR 13 (at 3:45). Patient reported feeling better after vomiting. Observation nurse took vital signs, offered water and Gatorade. Pt left our site at 3:50 (Patient's mother was driving).

Client received Pfizer #1. Five minutes after vaccine, client felt dizzy and lost consciousness briefly. Client became profusely diaphoretic. Transferred via wheelchair to medical cot. BP monitored over 20 minutes. Symptoms resolved and able to leave clinic independently.

PT felt dizzy and lightheaded after injection. transported to healthcare facility with father. vitals monitored. 1652 LYING - BP: 91/64, HR: 71, O2: 98 1659 LYING - BP: 93/68, HR: 80, O2: 97 1701 LYING - BP 101/71, 76, O2: 98 1706 STANDING - BP: 111/81, HR: 79, O2: 99

Patient collapsed to the floor and sustained a seizure <1min. patient states LOC. parent of the patient states no prior hx of seizure.

26 hours post injection, my son experienced a fever of 99.9, chills and fatigue. He then walked upstairs in our home and passed out-he was out for around 30 seconds. Afterward, his temp was still 99.9 and he began to experience vertigo. The vertigo lasted until Friday May 21.

Vaccine given at 5:19 pm. After the vaccine, mom took pt and walked around the store rather than stay in the waiting area. They returned at approximately 5:45 pm, because pt was complaining of a bad headache rating pain 6/10. He denied any other symptoms. We had him sit down in the waiting area and gave him a bottle of water. He started feeling lightheaded a few minutes later and was moved to an exam room. I had him lay down on the exam table. At approx. 5:55pm, I checked his BP while he was laying down and it was 104/62. He reported feeling a little better, so I had him sit up. Rechecked BP and it was 108/70. He reported dizziness had resolved and headache was now 5/10. At some point while I was not with the pt, mom gave him some Aleve, but I don't know what time. Mom decided to take him home at that point.

Fatigue, pain at injection site and soreness under left arm starting around 11pm on 11Jun2021; fatigue and underarm soreness and still continuing today (14Jun2021); dizziness starting around 6am on 12Jun2021 and still continuing today (14Jun2021); fever and chills starting around 8am on 12Jun2021 and subsiding by around 9am on 13Jun2021; raised, red/irritated skin around injection site, in shape of band-aid that was applied after injection, observed 13Jun2021 and still persisting as of 14Jun2021; increased dizziness and racing heart beat starting morning of 14Jun2021 and still continuing.

Patient received the 2nd dose of vaccine and he fainted 3 - 4 minutes after. His blood pressure was after 3 measurements over 15 min at an average of 98/58 with a pulse of 70. He also experienced a little dizziness, EMTs came at 2:20 pm and repeated the BP/pulse monitoring for about 20 min. Patient said that at one point felt cold and week but he was able to walk to the car with paramedics. No other symptoms mentioned during observation. Mom was counseled to monitor the patient and call MD if any new symptoms.

Patient felt faint while standing and singing during church. Passed out for a few seconds. Skin was hot and sweaty. Removed his jacket, gave him water and fed him a donut. Patient recovered after about 10 minutes.

Patient complained of nausea and dizziness after vaccine. Patient was sat down and BP was taken. Pt allowed to sit for 20 min in the flu shot room until recovered. Reaction was non-allergenic and attributed to anxiety.

Felt faint and nauseous five minutes after injection. Skin felt hot and sweaty. Said his stomach hurt and he felt like he was going to faint. Removed his jacket, sat him down, fed him crackers and water and he recovered after about five minutes.

Patient experience a syncopal event 4 minutes after vaccine was administered. Patient reported feeling lightheaded and passed out. Mother stated that patient has frequent syncopal episodes after vaccinations and blood draws. EMT/Paramedics obtained vital signs. Vital signs 133/84, 53, 20, 100 % and BS 134. Patient alert and oriented after syncopal episode. EMT/Paramedics transferred patient to Hospital ER.

After the shot given by (tech), within a few minutes the patient felt dizzy, blurred vision, then stomach felt irritated and patient started to throw up. At this moment when I saw the patient, he also had nose bleeding. While sitting, then patient stood up and was then asked to stay on sitting position. Soon patient stopped throwing up and right away felt much better. Within 15 minutes of observation patient return to normal and felt fine. The patient had breakfast right before the shot.

Systemic: Confusion-Medium, Systemic: Dizziness / Lightheadness-Medium, Systemic: Fainting / Unresponsive-Medium, Additional Details: patient received 2nd vaccination, did not sit and wait around 15 minutes as directed and fainted in the store. patient was very nervous before getting shot and stated later that he was sick last week and had not eaten anything all day (shot administered at 1230p). pt felt dizzy and lightheaded before fainting. paramedics were called and pt vitals were checked and all normal. pt went home with dad to be monitored closely for rest of day.

5 minutes from getting the vaccine, patient felt dizzy and looked very pale, there was remarkable pallor on the patient's face. skin looked pale.. initial blood pressure reading was 140/72. blood pressure upon discharge was recorded @ 128/82. Temp was 97.8 'C. Patient was given glucose tablets and few seconds after, patient felt better, skin suddenly turned pinkish.. patient recovered. Patient was monitored for 30 minutes, and stable upon discharge.

Patient has a history of Vasovagal Syncope and felt like he was about to pass out but felt better after lying down immediately.

Pt left vaccination room and roamed around the store around 11:00am. About 5-7 minutes later, Pt suddenly felt lightheaded and stood against the wall. Pt then got dizzy and sat down and was laid down on the floor by father. Pharmacist arrived to check on pt. Blood pressure for pt was found to be 96/67 with a HR of 63. Pt provided water and sprite to help bring sugars up since pt stated skipping breakfast in the morning. Pt started feeling better when paramedics arrived to check on pt. Paramedics cleared pt to go.

Had the vaccine on Saturday and on Tuesday afternoon, felt very dizzy, became very pale, had chest pain, pressure in chest and couldn't take a deep breath, vision kept moving in and out, had to sit down, felt very hot and cold at the same time and was actually sweating and feeling chills. Took it easy for the next 4-5 days and just stayed home and in bed. When it got really bad, took a dose of ibuprofen. The symptoms slowly got better and disappeared. Took about three doses of ibuprofen total over a few days. Talked to our pediatrician a week later who said we should report this.

At 2:45 pm I was informed that a 12-year-old who had just been given the vaccine was dizzy. I went to get a cotton ball with alcohol and immediately went out to assist him. After a few minutes he indicated that he felt better. The nursing graduate came out to check it again and was dizzy again. His arms were raised, ice was placed on him, then we put him on several chairs with his legs raised by pillows and 911 was called at 3:01 pm. The patient presented a complaint of tingling in the face and arms. The pressure was taken (126/76) Pulse 84. The father requests a stethoscope because he feels the young man's heart agitated and an oximeter since it also indicates some difficulty in breathing. Oxygen is placed and oximeter reflects a saturation of 99%. BP: 124/65. 911 is called back at 3:20 pm. They arrived at 3:26 pm. Paramedic indicated to give him liquid Benadryl. He was given 10ml. They proceeded to take the young man to the Hospital.

yelled out loudly when he received the vaccine but was fine immediately after. He was sitting for about 5 minutes before his mother said that he was feeling dizzy. The doctor was going to grab an ice pop for him when he fainted. was there as he was sliding off of the chair and had him in her arms. He then began to have a tonic-clonic seizure that lasted approximately 1 minute. The mother called 911. He woke up and asked what happened. The ambulance arrived and suggested that he go the hospital. The mother was on the phone with his father and he said he did not think an ER visit was necessary. The ambulance left and after waiting another 30 minutes, mom took home.

Patient became light headed and fainted from a seated position and fell on floor within one minute of receiving vaccination. Patient checked by pharmacist on site as well as Nurse practitioner and paramedics and was deemed well and fit to go home. There was a facial injury from the fall.

After receiving the injection, within about 5 to 10 minutes patient felt dizzy and fell in his fathers arms. Patient did not pass out but was dizzy, clammy, sweaty and pain in his left arm at injection site. I called 911. 911 came within a few minutes since they are stationed across the street from us. The EMT took his vitals. Vitals initially low at 88/55 then climbed back up to 102/68. No fever. Patient started to feel better. EMT stated patient is ok and may have

pt complained of getting dizzy and having trouble seeing when talked to him after. patient had parent with him (father) pt was nervous. got vaccine and left room. father said he looked very pale and then pt sat down and started to throw up. patient wasn't hospitalized. mild redness at injection site. left the premises after 15 to 02 minutes feeling fine.

Came back to consciousness after 5 seconds- stayed on chair whole time never hurt himself just a vagal reaction to vaccine. Paramedics were called to examine him and brought him to hospital for monitoring with mom.

After administering vaccines patient stated he felt dizzy and started to become pale. Applied a cool compress to back of neck and laid patient down. Patient never lost consciousness. Gave him some water and notified provider. VS taken and WNL Gave him some trail mix and had him sip on water. He stated he started to feel better. Had patient wait in office for 15min and patient was able to stand up and color came back and stated he felt better.

He was cognizant and able to stand on his own. Mother refused transport to the hospital and the family left about 45 min after immunization was given.

Pt felt dizziness and fell on the ground but did not lose consciousness. Symptoms went away after 2 minutes. Pt was assisted to stand up, seated and observed for 45 minutes. Pt felt fine and he was sent home. Father declined going to emergency room.

patient was sitting on bench in front of pharmacy and fell out of the seat onto the floor. Patient became very dizzy, flushed, and vomited. This happened about 10 minutes after vaccine given and lasted for about 10 more minutes. After throwing up, patient drank plenty of water and rested in which they started feeling good enough to walk away with their parent.

Patient was vaccinated with first dose of Pfizer vaccine within the pharmacy lobby of clinic then fainted seconds later. He was caught by staff member and transferred to a chair with the assistance of father and staff member; the patient was noted to have involuntary shaking of extremities. Rapid response was alerted. Patient regained consciousness within a minute and informed staff that he started feeling lightheaded immediately after receiving shot. Episode was witnessed by father and two pharmacy staff members. Patient was transfered to clinic, then discharged with an appointment set up with a cardiovascular doctor at the hospital to follow up on previously known heart murmur.

A 12-year-old male patient received (PFIZER-BIONTECH COVID-19 VACCINE, formulation: solution for injection, Batch/Lot number: EV00191) via an unspecified route of administration in left arm on 22May2021 at 11:00 (age at vaccination 12-year-old) as dose number unknown, single for Covid-19 immunization. The patient's medical history was none. The patient's concomitant medications were none. The patient previously took Flu vaccine, 2 years before, onset Age was about 10 years and experienced flu symptoms. Patient was healthy, no allergies and no medical history. Not even regular vitamins. Patient did not receive any other vaccines on same date of the pfizer suspect. Patient did not receive any other vaccines within 4 weeks prior to covid vaccine. Patient's mother was a bit concerned. He was doing great and then on 23May2021, Sunday onwards he had chills. Then he had a temperature of 100 and she gave him plain acetaminophen and Sunday onward 6 or 7 hours onward he kept getting chills. On 23May 2021, for six to seven hours, he had chills. There have been temperatures of 90 and she has to give him a blanket or heater. It was more or less the same today 26May2021 Attempted to clarify that the caller stated the intensity was a little less. Caller confirmed. She thought it was a normal reaction and waited patiently for 3 days. She was not allowing the temperature to go up as soon as he gets chilled and breaks into fever, she checked his temperature and it was always less than 100 like 99.9 or 99.7 Fahrenheit, axillary. Then he broke into a sweat and feels a little better. He also had tiredness and loss of appetite on 24May2021. She also reported that on 22May2021, his arm was sore and difficult to lift and then it disappeared in one or two days. He took the vaccine on 22May2021 in the morning and then by Monday symptoms were gone, the soreness, and warmness at injection site. Patient experienced tiredness on Monday 24May2021 in the morning after fever, the lack of energy and tiredness started. On 24May2021 patient experienced loss of appetite. His breakfast, lunch or dinner reduced to one fourth. On 22May2021 in the morning started in the evening, around 6 or 7 o'clock, patient experienced soreness at the injection site. On 22May2021 in evening, patient experienced warmness at injection site. The time range for adverse events was 3 to 5 days. 3 days of fever no one gives her answers. Pediatrician says wait and after 4 or 5 days they look into it. Adverse events did not result into emergency room visit and physician office visit. Treatment received for chills was gave acetaminophen every 6 to 7 hours. Treatment received for fever was Tylenol every 6 to 7 hours. Treatment received for tiredness and lack of energy was keeping him hydrated and his appetite was quite less and she was giving him lemon juice keeping fluids. Treatment received for event warmness and soreness at injection site was gave him ice and it helped a lot localized. Patient did not receive any treatment for event loss of appetite. The patient underwent lab tests and procedures which included fever was 99.9 and 99.7 Fahrenheit on 23May2021. The outcome of event chills was unknown. the outcome of event pyrexia, tiredness, loss of appetite and lack of energy was not recovered. the outcome of event soreness and warmness at the injection site and difficult to lift his arm was recovered on 24May2021. No follow-up attempts are possible. No further information is expected.

I administered the Pfizer COVID vaccine to the patient at about 2pm. I completed the vaccine, put the used syringe into the sharps container, grabbed the bandage, and turned back toward the patient. As I did this, he started slumping over in the chair, and his eyes rolled back into his head. I immediately put my arms around him so he wouldn't fall out of the chair. The patient's father was in the room as well and knelt down in from of him. The patient's body spasmed for a few seconds, then he came to but was breathing heavily and starting to sweat. I called to technician in the pharmacy and asked him to call 911 at 2:03pm. I brought the patient a cool cloth and asked a technician to bring some water. The patient's breathing started to slow, but he was nauseous and dizzy. He dry-heaved once, and I put the garbage can in front of him. He slumped over the garbage can for a few minutes. Then he asked to lie on the floor. He seemed to do better lying on the floor. When the EMTs arrived, they attempted to have him sit in the chair again, but his dizziness returned, and he complained of being cold. The EMTs moved the patient to the gurney and transported him to the Hospital for further medical attention.

Patient received vaccine and began to feel nauseated and lightheaded. Patient began throwing up. EMT responded. Vitals BP 118/98 O2 sat 98%. Patient and mom refused EMT accompany to hospital. Will wait in observation for 30 mins.

Patient received the vaccine and was completely fine immediately following. We walked around the store, conversed, and 10-12 minutes after injection he had syncope and generalized tonic clonic seizure for minutes. We were transported 911 to Hospital ED where he had normal labs, normal brain MRI, and consultation with Peds Neuro. He was post-ictal for 30+ minutes to follow the incident. He has secondary trauma to his R temporal and occipital bones and likely a mild concussion resulting from the fall.

pt had just received vaccine and found by ems crew with father at check out station. pt was seated with head in hands with complaints of light headedness and dizziness. pt was pale. pt was brough to evaluation area. pt suffered brief vomiting event. initial vitals were bp 85/50 hr 69 spo2 100% ra rr 18. pt was believed to ave been suffering from vaso vagal event.pt apperaed to recover quickly. pt second vital set were 90/60 hr 69 spo2 100%RA RR 18. pt skin color returned to a pink color. pt began to "feel much better" pts father gave consent to all evaluation and treatment. pt was believed to be stable. pt care was released. pt father was informed to contact emergency services if any symptoms returned and/or worsened.

Pt was feeling faint and lost color for a minute. We gave him juice, water and he ate a snickers bar. After 15 minutes he was feeling better and left with his mom.

Pt was pale and light headed. Syncopal event- caught by staff (did not fall). Hx of syncopal events with other vaccinations. Pt did not eat breakfast. BP 98/49, HR 65, RR 18 -- > 94/48, 83, 18 -- > 93/58 -- > 106/60. Pt laid down with feet elevated. Pt given crackers and juice. Pt improved, stable, and released from the vaccination site. Recommended patient eat and drink prior to next dose.

Spoke with father about 20 minutes after patient left and he said he was ok. He said he will follow up with us if anything changes.

Patient began feeling lightheaded, describes fuzzy vision and feeling tired about 2mins after vaccination. He requested to lie down. Appeared pale. Assisted to cot by medical staff. Vital signs checked within normal limits. He began to feel better after 5mins. Accompanied by mother who drove him home after 30mins observation.

Patient had a syncopal reaction: skin became hot and clammy, patient was dizzy, upon asking him to repeat a command or do a simple action (like wiggling fingers, lifting arm) he was not responding and his eyes were rolling to the back of his head and would not vocalize (when asked if ok, or what his symptoms were, or if he was having trouble breathing) at which time we called 911 since he was not responding verbally or physically. Had him sit, take off his mask, gave him water to sip, took BP (102/60), within 5 minutes he started to come back around, was able to follow command (wiggle fingers) vocalize that he could breathe fine, and felt that there was water behind his ears. His temperature to touch seemed to start to go back down, and once paramedics arrived they evaluated him and released him as "ok" within 10 minutes.

Patient received dose of vaccine. While standing patient experienced syncope and fell to floor. Bumped head against wall while falling but was caught by father before hitting floor. He quickly regained consciousness and had a strong pulse. He remained on the floor with feet elevated for 10 minutes. He then sat in chair IN front of pharmacy for an additional 15 minutes. At that point he felt fine and left the pharmacy with his father.

BP 102/60, P 70, O2Sat 98%. As client walking to the observation room he passed out for a few seconds. Complained of dizziness. Assisted with ENT to observation. Stabilized after few minutes.

The pt was awake but felt light headed, had low blood pressure and blood sugar. The pt was given water and crackers. My DM was also here and called the paramedics who took him to the hospital.

Code assist called- Pt complaining of dizziness- blurry vision- Took pt's BP and it was 168/120-2nd BP 65/28- RN's and MD's assisted the code and 911 was called.

after administering vaccine patient started to vomit and drop in his pulse, dizzy and bit shakier. after observation called 911 and paramedic came and after checking everything falls within normal limits and patient went home.

Patient developed SVT (supraventricular tachycardia) approximately 29 hours after receiving the COVID vaccine. Patient has no prior history of SVT or any other cardiac related events. Patient was engaged in a basketball game when the SVT occurred. Patient has been involved in multiple competitive sports since the age of four without any prior incident related to exercise. He felt like he could not breathe, his heart was racing, he became dizzy, began to lose his vision and I took him to the local fire station for evaluation.

Pt received vaccine in left upper arm. He was bleeding immediately more excessive than a usual bleed after shot. no medications history. he felt faint also. applied cold compress to head and mom applied pressure to arm with paper towel. bleeding stopped and he was feeling better . Checked in with mom on patient at home and he was feeling better

Pt c/o lightheadedness, dizziness, flushing/sweating, nausea. Pt vomited 200mL digested food. Pt given PO fluids. BP 93/42, HR 66, RR 20 -- > 105/57, 59, 18 -- > 108/60, 65, 16 -- > 109/68, 68, 16. Pt was COVID positive Jan 2021 (s/sx body ach). No h/o fainting with vaccines or blood draws. Pt's symptoms resolved after drinking water and laying down with knees bent. Advised taking 2nd dose supine.

sycope, pt started experiencing fainting symptoms such as sweating, paleness, hearing disturbances, weakness, dizziness, coldness of extremities after about 3 minutes of the vaccine, symptoms resolves in about 20 minutes.

At first patient was fine just sore arm. Approx 2pm the next day we were at Aunt's house & he became very sleepy and fell asleep outside. I nudged him to see if he was okay and he couldn't lift his head up all he said was "I hate you" (he didn't want the shot and I had a feel well) I kept nudging him to go inside and he didn't react. Finally after 25 min I had my brother in law carry him upstairs. He didn't wake up for it (the kid is 13 that never happens). Every hour I checked on him and he wouldn't wake up. I am a nurse do figured the shot made him tired but I did start to get anxious I even sternal rubbed him and he reacted to pain but would not wake up. Finally around 445 I told my husband we need to make him drink something and I stood him up he would not wake up. I was trying everything but we got him up enough to gulp down some gatorade. We put on his sneakers and he basically sleptwalked down the stairs went right to car and fell asleep again. It was like he was too tired to stay awake. We got him home and around 515 kept him sitting up in kitchen. Took him about 20 min to fully open eyes and wake up. When he was fully awake he said it was like the shot moved from his arm to his whole body when it happened and that was it. He was sore and achy after that and even today to generalized body. But he was fine as far as not lethargic after.

Patient lowered to floor by staff after feeling faint. Put in trendelenburger with feet up until less faint. Remained in seated position for approx 20 min w/observation.

Pt was feeling lightheaded approximately 3 minutes post vaccination (1st dose of Pfizer). Pt then had syncopal episode while remaining in chair. EMS on site and was immediately alerted. EMS put pt on stretched and brought to ambulance for further assessment. Pt observed by EMS for 15 minutes and then went home with Dad. Per Dad pt has history of syncopal episodes post vaccination.

Vertigo starting 4 days after the shot with no other symptoms up to then. The vertigo exhibited as nausea when moving head that resulted in vomiting. Any movement of the head results in dizziness and nausea. No other symptoms in the rest of the body, no fever, no headache.

very dizzy starting 05/19/2021 upon awakening almost one week since covid vaccine; pt switched his clonidine paltch to a new patch and put it on the same arm that received the vaccine; over the area where received his shot and within 2 hrs began to feel very sleepy and very dizzy ; went to dr yest and bp 82/58; dad to provide more info so that VAER can be done it was a new box of patches ; looked same as always ; took patch off 5/19 around 530 pm; seemed to wear off 30min later; and started to feel better and less dizzy ; and tic symptoms increased. Needed to take 0.1mg tab at bedtime to sleep next day dizziness "barely there" has been drinking approx 4 waterbottles per day and eating well

Pt became pale, diaphoretic, and nauseous. Vomited undigested food once and complained of dizziness. Pt was laid down on the floor with his feet up on a chair. His color immediately came back, and he felt better and went home.

Patient was given the shot at 9:36 am and was asked to sit down for 15 minutes to watch how he reacts with the vaccine. He had been very nervous and anxious before getting the shot because he never had a shot before and was afraid of needle. After around 5 minutes, his dad notified me that his son had upset stomach. I asked the patient if he ate or drank anything in the morning. His dad and he confirmed that he didn't eat or drink anything (because dad thought that he should not eat anything before getting the shot like getting blood drawn). He confirmed that he breathed okay and didn't have swelling of lip and throat. His father told me that his son did feel a little bit dizzy and sweating. Since the clinic is full of patients and humidy, I told him and his dad to sit in a room in clinic without patients and bring a fan to the room. After 15 minutes (around 9:51am), after drinking water and eating a banana, all the side effects were gone. He felt better, and he could watch videos/games and have full focus. At 10:24 am (which was more than 30 min resting in clinic), his father and he confirmed that he was feeling better and didn't have any side effects and decided to leave the clinic and go home.

5/15 vaccination 5/25 He got up and go dressed, ate breakfast, was ready to do work out in yard. Went to walk across the room to his brother, turned to look out the window, and all of a sudden fell straight back. Came to 10 seconds later, didn't know where he was, felt like he had had a bunch of dreams, 'everything turned white', doesn't remember much, didn't know where he was (thought he was in bed). He had hit his head on the wood floor, back of head (standing to smacking head). We live on an island, had to take ferry to town. Possible concussion. We went to Hospital in town. We worked with Dr. They gave us the verbal instructions if HA gets worse, anti nausea meds, rest, ibuprofen; concussion.

Patient returned to baseline.

Sudden chest pain radiating toward armpit; Dizzy; Sweaty; Rapid pulse rate; Nausea; This is a spontaneous report received from a contactable consumer (patient). A 13-year-old male patient received first dose of bnt162b2 (PFIZER-BIONTECH COVID-19 VACCINE, Formulation: Solution for injection), via an unspecified route of administered in left arm on 21May2021 13:30 as single dose for COVID-19 immunisation at School or Student Health Clinic. Medical history was reported as none. The patient did not receive any other vaccines within 4 weeks prior to the COVID vaccine. Prior to vaccination, the patient did not diagnose with COVID-19. Since the vaccination, patient had not been tested for COVID-19. The patient did not receive other medications within 2 weeks of vaccination. The patient's concomitant medications were not reported. Patient had no allergies. On 21May2021 22:30, the patient experienced Sudden chest pain radiating toward armpit, dizzy, sweaty, rapid pulse rate, nausea. The symptoms resolved after about 10-15 minutes. This repeated a second time on Saturday 22May around 4 pm. Repeated a third time on Sunday 23May around 3 pm but was less in severity. Lot number on card not clear but appears to be EN0185 or EW0185. The events assessed as non-serious. No treatment was received for the events. The outcome of the events was reported as unknown. Information on the lot/batch number has been requested.

Patient became dizzy/started losing his vision fairly quickly after receiving the vaccine. He kept repeating he couldn't see. He stumbled forward then fell down. Patient did hit his head. Store associate got him up and into a chair and started asking him questions. Mom stated he had just received vaccine. They were not told by the pharmacist on duty to stay to be monitored for 15 minutes after. He did say that he had a headache. Gave the patient some water, had him move his arm around, and monitored patient until pharmacy closed.

Client received the 2nd COVID vaccine of Pfizer (Lot# EW0182, expiration 06/28/2021). At 1729, Client reports nausea and wants to vomit. RN, RN, RN, RN, and RN respond to this event. The registered nurses placed Client in an anti-gravity chair and provided an emesis bag. RN responded to this event at 1730. Client appears pale, diaphoretic, states being nausea, wants to vomit, and dizzy. At 1730, vital signs are heart rate 89, oxygen saturation 99%. At 1733, vital signs are blood pressure 110/82, heart rate 62, oxygen saturation 99%. Client is offered supplemental nutrition and fluids. Client accepted both. At 1735, heart rate is 61 and oxygen saturation is 99%. At 1735, PHN responds and speaks to mother to educate her about ED precautions and common adverse effects. Client's mother reports Client has a history of ADHD, takes medications as needed, no known allergies, had a long day, as not ate much today, and was nervous prior to vaccination. At 1736, vital signs are blood pressure 110/78, heart rate 66, oxygen saturation 99%. Client reports no chest pain, no blurred vision, no headache, no shortness of breath, nausea is improving, and skin tone is returning to appropriate coloration of ethnicity. At 1742, vital signs are blood pressure 117/82, heart rate 82, oxygen saturation 99%. Client reports no new symptoms and improvement of nauseousness. At 1753 vital signs are blood pressure 112/68, heart rate 86, oxygen saturation 99%. Client reports all symptoms has resolved and is now hungry. Client stood up with no complaints at 1754. At 1754, Client walked out of facility with a steady gait accompanied by his mother.

Pt felt feverish and lightheaded within 5 minutes of vaccine. He experienced sweating and chills. Administered one dose of acetaminophen. Pt was monitored for 30 minutes and felt better prior to leaving. We followed up later in the day and he did not experience any further symptoms.

Pt c/o dizziness immediately after receiving 2nd dose of Pfizer. Pt had one episode of syncope. Father was present and assisted pt to the ground. No falls. Pt reports feeling nervous. A&Ox3. pt speaks in full complete sentences. No facial/mandible/tongue swelling. No stridor. Pt ambulatory to med bay, refusing to sit in w/c, w/ steady gait. Pt refusing pulse oximeter. Resp regular/deep. BP stable 149/50, HR 88, RR 16. Pt tol PO. Pt stable and released from the vaccination site. Recommended that the patient follow up w/ their healthcare provider.

After getting vaccinated , pt sat in the waiting room for 15 minutes. While sitting he fainted and another customer caught a hold of him. I was called out and I see the boy on the floor . I told him to sit still and rest and not to do anything. I went back in pharmacy to call manager to see if we can get some water for him. I also had someone page mom because mom went shopping. When I went back out to see the boy, his mom was there and he was drinking some water. I came out to talk to mom. while I was there, the boy started to shake and complained of chill. He look pale, so, I went and called 911. They transfer me to the fire department. They said to have him stay where he is and not have anything to eat or drink just incase her may throw up.I told mom. After that , I went about doing my things . Then emergency came . Then they all left. About 7 pm that day I called mom for follow up and she said he was doing ok.

Immediately following the vaccine, my son felt totally fine. The pharmacist told us to walk around the store for 15 minutes. 8 minutes after the vaccine, my son said his arm felt tingly. He immediately began sweating profusely and said that he felt dizzy. I made him sit down and he then said that he felt nauseous. Within 5 minutes of the onset of symptoms, he vomited. He was pale and his forehead was cold/clammy. The pharmacist said it was anxiety and nothing to be concerned about. However, my son said that he did not feel worried, anxious, etc. prior to or immediately following the vaccine.

Patient was disoriented , pale, and complained of blackened vision and huge sound in his ears. paramedics were contacted and patient was arousable. after 15 min patient was ok and documented color restored to complexion. paramedic deducted anxiety due to needles and patient was taken home by parents

13 year old male presents with chest pain. Pt describes pain as achy. Pt feeling dizzy "seeing stars" and has headache as well. Aches located on both arms and neck. Chest pain worsens with deep breathing. Neck pain worsens with neck movement. Pt had second dose of Pfizer 2 days ago; parents are concerned because a relative contracted myocarditis 5 days after receiving vaccine. No recent chest trauma. NBNB Vomit 1 month ago. Taking ibuprofen at home. No fever. Differential Diagnosis: flu like illness, post vaccine syndrome, myocarditis, doubt meningitis, myositis. Cardiology consulted, Agrees with workup and add CMV and EBV antibodies as well as enterovirus if elevated troponin. Troponin = 6.84. Will be admitted to CVICU.

About 5 minutes after administration pt felt lightheaded, dizzy, and nausea. Gave pt water and smelling salts.

Vasovagal syncope - While seated in chair, patient became dizzy and glass-eyed approx 10 minutes after vaccination. Before he could fall, he was lowered to the ground and positioned with legs raised. Cool, wet cloth applied to patient's forehead and ice pack applied to back of patient's neck. He was also give cold water and orange juice to sip on. Remained supine for 15 minutes, then sat in chair for another 10 to 15 minutes before going home with Mom (a nurse). Followed up with patient's mother @ 7:45 pm - patient was fine.

Felt 'funny', slightly dizzy Initial Treatment: Arrived at clinic alone and riding bike. Gave water, juice, and snacks prior to vaccine. 4:20 Vitals: BP 110/80 HR 102 O2Sat 98 Skin Signs: Warm and dry Follow up Treatment: Water, snacks, laying down Doing well on discharge. Dad's friend came to get son (cleared upon phone call with Dad). Stable and no symptoms on discharge.

On 05-20-21 We received a text message from the pts mother stating that the pt was experiencing drooping of the face , pain in jaw all on left side and dizziness.

Patient received covid vaccine (Pfizer) on 5/18/21. About 2 weeks prior to admission to hospital patient began having leg weakness and unsteady gait which has progressively gotten worse. Patient is beginning IVIG treatment for Guilian Barre syndrome.

13 year old male received Intramuscular Pfizer vaccination (Lot# EW0191) in right deltoid on June 9th 2021 at 12:44pm . At approximately 1:00pm the client complained of headache and dizziness to his guardian. (Name) who has her BSN in nursing and her interim permit at observation went over to assess the patient with other onsite staff. The patient denied difficulty breathing. Patient was oriented x 3 with stable vital signs. Vital Signs were taken at approximately 1:00 pm Pulse 58; RR: 12; B/P: 113/67; O2: 100%. Second B/P was taken at 1:19 pm and was 129/79. Client stated he ate breakfast today. No medication was given. Denied having difficulty swallowing. Water was provided. The client stated he was feeling better and after waiting an additional 20 minutes left home with his guardian.

Systemic: Dizziness / Lightheadness-Mild, Systemic: Flushed / Sweating-Mild, Systemic: Hypotension-Mild, Systemic: Shakiness-Mild, Additional Details: Patient felt dizzy, unstable upon standing, skin became pale, sweaty, cool and clammy. Hospital treated for potential shock - moved from sitting in chair to reclining position with legs elevated, cold compress applied to back of neck. BP was low upon initial reading, but improved to within normal limits after 10 minutes. Patient remained under observation for additional 30 minutes. Follow-up phone call to patient's mother (patient is minor) by hospital, mother reported no additional problems

The patient started falling over, our staff caught him so he did not fall out of the chair. His eyes were rolling back, he was shaking and his throat was making abnormal movements. His mother rushed over and bear-hugged him, and its like he came back to reality and was okay. We got him a cold pack, water, fruit snacks, a mat to lay on and a fan and the patient walked out okay. His reaction was directly after the shot and lasted about 5-10 seconds. Patient walked out of the facility fine and had his mother with him.

Unspecified intracranial injury with loss of consciousness of unspecified duration, initial encounter Fall while sleeping two days after receiving vaccine injured finger and head

Pt received dose 2 of Pfizer covid vaccine. Pt moved to recovery area and sat down. Pt lost consciousness and fell from chair onto floor, landing on right side of face/head. Vitals BP 84/49, HR 59, SaO2 100% RA, resp 16. Pt regained conscious and his feet were elevated on a chair by staff. EMS arrived and pt's mom transported him to ER.

Brief 1-2 second episodes of vertigo once or twice daily since day of vaccination, reported on 6/10/2021. Of note, patient had one such episode of vertigo one week prior to first vaccination on or about 5/13/2021, so not entirely clear that this was new symptom after vaccine, but frequency did increase. No other associated cardiac or neurologic symptoms.

Patient was hyperventilating before vaccine was administered. After vaccine was administered patient started to get dizzy and skin color changed. Patient lay down, vitals were being taken, and water was given. once patient smelled ammonia patient started feeling better. Patient stayed for 15 minutes to ensure he was stable to go home.

Was unable to move hands, wrists, feet; Faint; Temp dropped to 95; This is a spontaneous report from a contactable consumer (patient). A 13-year-old male patient received the first dose of BNT162B2 (PFIZER-BIONTECH COVID-19 VACCINE, Lot number and expiration date were not reported) on 22May2021 (14:00) as a 1st dose, single dose, with route of administration unspecified, for COVID-19 immunization at the pharmacy/drug store. The patient had no relevant medical history and concomitant medications. On 22May2021 (14:00), the patient was unable to move hands, wrists and feet; fainted; and temperature dropped to 95. The events were assessed as serious (medically significant). The events had resulted into emergency room/department or urgent care). The patient did not receive any treatment for the events. The outcome of the events was recovered in May2021. Prior to vaccination, the patient was not diagnosed with COVID-19. Since the vaccination, the patient had not been tested for COVID-19. Information on the lot/batch number has been requested.

Patient was examined by his pediatrician and cleared for physical exercise on 06/01/2021; 4 days before receiving his 1st COVID19 shot on 06/05/2021. On 06/07/2021 we dropped patient off at Soccer Camp. At 10am he started basic soccer ball control drills, which did not include rigorous exercise. At 10:30am we received a call from him, asking for us to pick him up as he had almost blacked out a few times. We arrived at the sports field at 10:40am. I found patient to be red faced, sweaty, but cohesive. He described the following symptoms occurred during mild exercise: difficulty breathing, thirsty, fast heartbeat, "black things" over eyes, preventing clear vision, difficulty focusing, difficulty hearing, feeling faint He took a water break and used his pro-air inhaler, as he thought he might have been having his 1st asthma attack (he's only ever presented with slight wheezing or night time cough). After a short break, his symptoms subsided, so he re-joined the group. After one more soccer drill, the symptoms came back again, but this time he experienced very hard palpitations and the feeling that his shoes were squeezing his feet. At that point he asked for permission to call to us and have us collect him. We took him home and monitored him for a while. He seemed calm and symptom free. I had him shower and he was fine for the rest of the day. The following day I had him jog on our treadmill for a period. He was symptom free. I increased inclination on the treadmill and he handled it ok without any of the previous symptoms. However, now realizing that he was within the 3 day window of his COVID shot, I decided not take him back to the soccer camp. As of 06/13/2021 he has not complained of any symptoms returning. That said, he also has not performed any significant outdoor exercise since then either, besides walking.

Felt faint, was brought over to adverse reaction area, sat in a wheel chair, vitals taken. Initially, elevated heart rate, 118, bp of 92/58. By the time of discharge from clinic, vital signs were 100/54 and heart rate of 96. Patient was released from school to mother.

Patient received the vaccination and per the mother's account "seemed tense when he got the shot". The patient started to walk to the waiting area after receiving the vaccine when he dropped to his knees and reported feeling dizzy. Our physician observers noticed the patient was also sweating and quickly moved him to a stretcher to recover. After a short time of observation (30 min +), the patient was feeling well and sitting up eating and drinking water. According to our physician observers, the patient never lost consciousness and was not injured.

On the night of the vaccine, patient developed fever (Tmax 104), chills, and myalgias. He had an outbreak of herpes on his shoulder. Continued to feel very fatigued. Three days after the vaccine, he started having chest pain and shortness of breath with exertion. He continued to be fatigued. Chest pain was intermittent. Saw pediatric cardiologist at outpatient appointment on 6/15/21 where he had a normal EKG and ECHO. High sensitivity troponin on 6/15/21 was 0.08. He had a Holter monitor placed. He was asked to do activity while wearing Holter monitor and self-limit based on symptoms. He played basketball that night and had to press the Holter button 8 times while playing when he was experiencing chest pain. When he got home from practice, he felt weak and dizzy with associated chest pain. Mom notes his nails were blue/purple, and he was "lethargic" . Symptoms improved with ibuprofen. Patient did not want to do usual activity, mom called cardiology who recommended going to ED. Patient has not had chest pain since coming home from practice on 6/15/21. He was admitted to hospital on 6/16/21. Repeat high sensitivity troponin was 0.82 on 6/16/21 and repeated on 6/17/21 was 0.41. Cardiology consulted. Cardiac MRI performed with normal results. He was dizzy on 6/17/21 when he got up to use the restroom, self-resolved. Cardiology evaluated him and cleared him for discharge with outpatient follow-up and self-limitation with activity.

Fever; Dizziness; Headache; Night terrors; Slept all day; This is a spontaneous report received from a contactable consumer (Patient's mother). A 13-year-old male patient (reporter son) received BNT162B2 (PFIZER-BONTECH COVID-19 VACCINE, Solution for injection, Lot Number: EW0182) dose 2 via an unspecified route of administration on 11Jun2021 16:30 (at the age of 13-year-old) as single dose for covid-19 immunisation. Medical history included ADHD (attention deficit hyperactivity disorder). The patient's concomitant medications were not reported. No prior vaccines (4 weeks), no other medical conditions. Reporter Stated, he did not have any other medical conditions besides ADHD. Patient historic vaccine included first dose of BNT162B2 (PFIZER-BONTECH COVID-19 VACCINE, Lot number: EW0182) received on 21May2021 at 2:00 PM, administered in left arm via IM (intramuscular) route, and experienced night terrors and fever. On an unspecified date in Jun2021, the patient experienced fever, dizziness, headache, night terrors and slept all day. The patient, on Friday afternoon at 16:30 received his second Pfizer vaccination and he had the most he had common obviously he had the fever the next day, the dizziness, the headache, slept all day, he had the fever all day, around 6:30, he had a really bad night terror and he also had that with the first injection too. Reporter stated, she thought that he just had it with the first injection and that was gonna be it but she didn't think that he was gonna have the same results with the second injection cause he had it with the first one but he had it a really bad night terrors with the second one as well so he's pediatrician wanted her to report it again with the second one because his pediatrician hadn't been seeing that with any other children. She did not know if any other parents had been reporting these. When Probed if the patient was still experiencing these problems, reporter stated that, the symptoms were improved he did not have them since. Reporter Stated, she gave him paracetamol (TYLENOL) for the fever and the headache. It was 200 mg. The outcome of events was reported as recovering. Follow-up attempts are needed. Further information is expected.

Patient is currently being treated for Guillain Barre syndrome, with symptoms of bilateral feet weakness that then extended up to his thighs, leading to difficulty walking and falling with attempts to stand. He has had no loss of sensation and also states that he had sensation of a full bladder but could not urinate and did experience urinary incontinence.

After the vaccination, within 5 minute the patient felt faint and started to lightly vomit. The stayed seated and over the next 25 minutes, periodically vomited small amounts and eventually started to feel better and in better spirits.

Acute-onset pressure-like 3-5/10 mid-sternal non-radiating chest pain worth with inspiration associated with palpitations and light-headedness

Pt was lightheaded, w/ nausea, w/o any pain or vomiting. Patient transferred to gurney Assessment done and vitals checked. Patient stated that he had not had a good breakfast in the morning BP 94/60, pulse 62, 02 sat 98%. Advised him to drink sips of water, and snacks were given. He was observed for an extra 20 minutes. Patient felt better after 15 minutes BP 118/70, pulse 64. Advised patients father to consult with PCP before second dose. Advised to drink more fluid and keep hydrated. patient left for home with father without any symptoms of being lightheaded or nauseated

3 days post injection patient developed increasing dizziness, light headedness, headache, nausea, achey muscles, decreased appetite, shortness of breath with exercise (new complaint), slight cough symptoms persist still 11 days after injection

13 year old male patient who after two minutes of receiving the second dose felt dizziness, he appears sweaty and vomited once He sits in turn to get evaluated.

After patient received COVID Pfizer shot patient went to sit on stool in waiting room. Patient didn't sit on regular chair to sit closer to sister and mom. After a few minutes patient got lightheaded and fainted falling back and to the right, hitting head against wall and blood pressure machine and floor. Mom and sister tried to catch patient during fall. Patient had light shaking for a few seconds before becoming conscious again. I went out and called 911 for paramedics, lifted patient legs up to improve blood flow to head. After a few minutes and after finishing call with 911 had other employees help lift child onto regular chair to sit. Patient got a small bottle of water to drink. Paramedics soon arrived after and evaluated patient. Paramedics determined child fine and mom decided to take child home. Called mom for follow up a few hours later and child doing fine per mom.

Patient fainted and fell out of his chair. He hit is head against the wall on his way down. His sister called us over. The patient I was counselling at the register happened to be a nurse. She reached him first and assessed him for breathing and heart rate. He was not bleeding. We placed him in the recovery position. The nurse noticed some firemen in the store and called them over. They did a basic assessment, but not a full write up. After 10-15 minutes, he felt well enough to sit up. He remained seated for another 10-15 minutes. He said his head hurt and was a 4 on a scale of 1-10. I told the patient's father to monitor his headache and if it gets very bad to seek medical attention.

Received vaccine in clinic room. Eight minutes after receiving COVID-PFE vaccine, patient complained he was ready to go and felt drowsy. The temperature did seem warm in the clinic room, so I escorted them 30 feet to the lobby. As the patient passed through the door to the lobby, his gait, he seemed to lose his ability to remain upright and started to collapse. I was able to catch him prior to hitting the floor. He never lost consciousness, but we moved him into a clinic room to evaluate and further monitor him. His blood pressure was a little low initially at 110/50, but came up to 112/60 (closer to his normal) after five to ten minutes. His pulse was strong and regular. We gave him a cool, wet compress to the forehead, cool water to drink, and chocolate candy. He was able to answer questions appropriately. We monitored him for another 30 minutes before releasing him. His mother was present with him and is a nurse practitioner. I called the Mom on 6/22/21 (the day after the event). She (Mom) reports the patient went home, laid down for about 45 minutes, then played video games. I recommend she considers getting the second dose at either Hospital, so that a higher level of care is available if needed.

Patient became light headed and nearly passed out. Patient also vomitted twice. Patient was weak and feeling warm and clammy for about 15 minutes.

Patient (13 years old) came with his mother at 10:36AM for second dose of the Pfizer vaccination the Covid-19 clinics that are celebrated Wednesdays and the patient is not allergic to medications only seafood and the first dose there was no adverse reaction. When he was asked to wait for 15 minutes assistant that was in the Clinic informed me Pharmacist on duty that the boy felt dizzy and was vomiting. I went to their location with alcohol to inhale. We allocated the head between the legs. At 10:47AM blood pressure reading was taken BP= 112/70 P=101. Mother indicated he only had a juice for breakfast. At 10:52AM he was given liquid BENARDYL 10ml as anti-hemetic to vomiting. In this course we closed the treatment. As the patient was feeling better. We took another reading at 11:07AM BP=113/90 P=94. When the patient felt better 55 minutes he left with his mother. I called the mother of the patient at 4:18PM to know how patient continued and she told me after they left the patient went out to eat and was good and was sleeping. Assistant helped me.

15:08 - Attendee received second dose Pfizer vaccine in the left arm. Lot# ER8734. Attendee was directed to observation area. 15:10 – Attendee voiced complaints of dizziness and darkened vision. Attendee's skin was pale and clammy. Attendee denied that anything like this had ever happened to him before. Attendee assisted to the cot to lie down. 911 called. HR: 96 BP: 100/59 RR: 24 O2Sat%: 96 15:15 – EMS arrived. Attendee's brother who had come with him, called their mother. 15:22 - Attendee symptoms had not changed. EMS caring for attendee. HR: 92 BP: 108/70 RR: 24 O2Sat%: 98

Developed dizziness and fever the morning following vaccination followed by chest pain around 48 hours after vaccination. Presented to outside urgent care where pt noted to have elevated troponin >10, prompting transfer to ED. and admission for myocarditis. Symptoms improved over next several days and chest pain was well controlled with ibuprofen and tylenol.

5-10 minutes following administration of Pfizer Covid vaccine, patient ( 13 year old) complained of light headedness and the inability to hear with ringing in his ears and diaphoretic. Patient did not have any complaints of difficulty breathing. Patient became pale and lowered to floor with feet elevated. Blood pressure taken by RN at that 9:30 was 89/60 with HR at 60. Nurse was initiated and arrived at the clinic for assessment. Observation was continued with the ER team present. Blood pressure at 9:36 was 108/60 and then again at 9/40 was 100/68. Patient did state his symptoms improved and was transported to the ER via the ER team for continued monitoring with mother and his sister. Report given to ER Nurse.

Paramedics were called, but mother did not feel patient needed emergency service and will be monitoring her son at home. Called 2 hours afterwards, and mother reported son is feeling much better.

Immediately after receiving this vaccine the patient felt weak and lethargic. They lost balance and fell on floor and lost consciousness for a few minutes. After a couple of minutes they gained consciousness and recovered.

Systemic: Chills-Medium, Systemic: Confusion-Mild, Systemic: Dizziness / Lightheadness-Severe, Systemic: Fainting / Unresponsive-Medium, Additional Details: The patient felt dizzy about 3 minutes after receiving vaccine (after walking and sitting in observation area). Pharmacist ran out and caught patient as he lost consciousness and the patient was not injured. Patient regained consciousness after ~10 seconds. Pharmacist and Technician helped patient to lay on the floor and elevated feet. Vitals taken every 5 minutes. Lowest BP reading was 98/60. Patient offered water and drank some. Patient laid down for ~25 minutes, BP came up to 116/84.

Patient received his second dose of covid vaccine on 07/12/21. He seemed fine and left our immunization area. Approximately 5 minutes later, his brother came back to the pharmacy to inform us that the patient was sitting down near the restrooms and not feeling well. I, along with another pharmacist, went to assess him. He said he was lightheaded and was feeling nauseous. We sat with him for approximately 10 minutes and checked his blood pressure twice. His blood pressure was high the first time we took it, but it returned to normal after a few minutes when we took it the second time. We subsequently found out from his mother that he fell/fainted in the restroom due to his dizziness. He was unhurt from the fall. After approximately 10 minutes he said he was feeling fine and was able to continue shopping with his mother.

Patient stated he felt light headed and dizzy with the first Pfizer vaccine and requested to lay down to have the Vaccine administered. The nurse took the patient to a private area, with the mother and brother. The nurse vaccinated the patient at 1423, patient then had a seizure at 1425, I was called to assisted. Patient at the time was crying and felt sick. I asked for the nurse to place the patient on his left side while I obtained the necessary equipment for vital signs. Patient then became no verbal, I Immediatley asked for the staff to call 911. Patient was cool, diaphoresis, not responding. I sternal rubbed the patient as he was not responding. He was breathing, O2 saturation was 98%, HR 59. Vital signs 100/60 RR 16. Patient remained non-verbal. I asked the mother if the patient had any medical conditions and she stateed no. I asked about seizures and then the brother stated the patient had seizure when he was child about 3 years ago and the patient currently is not on any seizure medication. EMS was called at 1427 and are in route. Vital signs again, 110/78, HR 78, resp 18. I continued to ask the patients questions and the patient remained no-verbal. The patient followed directions, but remained non verbal. EMS arrived 1435 and took over. Patient did awake a little more and stated he didnt want to go to the hospital. EMS took the patient to the nearest Hospital

Following vaccination, patient experienced syncope episode and fell to floor. He regained consciousness very quickly but felt dizzy and nauseous for a little longer. Observed patient and he was feeling better prior to leaving pharmacy.

About 1-2 minutes after giving vaccination the patient fainted while sitting in the chair. We laid him on the floor. He was feeling light-headed, hot, and achy all over. He felt better after several minutes with a cold pack and had some water to drink. The paramedics were called to check him out. He was able to sit and stand and left after about another 15 minutes.

Patient received his first dose of Pfizer Covid-19 vaccine on 5/15/2021 and the second dose of Pfizer Covid-19 vaccine on 6/5/2021. After an abrupt onset of mid thoracic back pain and leg weakness on 6/16/2021, the patient rested for the night. In the morning, he was unable to walk, characterized by dragging his feet and falling o the ground. He was unable to void. Mother sought care for him at the Emergency room on 6/17/2021. In the ER, he had an MRI scan of his spine which showed no significant central canal foraminal stennosis. He was catheterized and had urinary residual of 600ml after an episode of urinary incontinence. Neurology at the Clinic was consulted and patient was transferr

Systemic: Fainting / Unresponsive-Medium, Systemic: Visual Changes/Disturbances-Mild, Additional Details: Pt informed to sit after receiving vaccine mom walked away then pt got up and started walking and passed out in aisle walking to mom and scraped his elbow. pt seated and had something to drink, felt dizzy and felt vision was blurry upon awake. Informed mom we could call paramedics but mom felt he was okay. Observed for 30 min after fainting incident, felt much better, vision no longer was a problem and no longer light headed. Mom told to call Dr. in case pt hit head, said to observe him

Mom states she found her son around 9 PM in the bathroom vomiting. She stated she moved him to the couch and he continued to vomit. She stated he fell off the couch and he was seizing. She called 911, when they arrived he was no longer seizing. When her arrived at the hospital, it was documented he was postictal and would not respond to painful stimuli. The hospital kept him overnight and are discharging him home today. He has fully recovered.

PATIENT EXPLAINED THAT HE HAS SLIGHT DIZZINESS DUE TO BEING NERVOUS ABOUT THE SHOT. THE PATIENT WAS SEATED AND EXPLAINED THAT HE FELT MUCH BETTER. THE PATIENTS PARENTS EXPLAINED THAT THEY WOULD TAKE PATIENT HOME AND MONITOR.

patient got dizzy a few minutes after the vaccine administration, fell on the floor but he was conscious and he was able to get up later on. We continued to monitor him. No emergency call 911 was made

Ringing in ears, feeling light headedness. Seen by EMS. Discharged home with mother at 14:30 . Recorded by RN

Pt had spent 10 mins in the observation hall when he w.. as reported to be feeling light-headed and his vision, blurry. Mother reported that he had nothing for breakfast b/4 the vaccination appointment. V/S taken was bp 98/57, rr 14, Hr 80, O2 96% RA, T. 95 degrees. When writer offered some drinks and snack, the Pt accepted the offer. 2 small packs of apple juice and 2small packs of crackers were consumed by the Pt. A recheck of the v/s showed the following readings. bp 98/64 , Hr 69, O2 99% RA .Pt and mother stayed for about 15 extra mins before the Pt verbalized feeling okay and ready to leave. He left in the company 0f mom unassisted

His neck lymph nodes are very stiff and swollen and it hurts badly. He says he has a hard time turning his neck

Patient verbalized dizziness and tunnel hearing. RN noted patient diaphoretic w/face pallor. Safely placed patient on the floor and elevated legs on a chair. O2 97-99%RA and HR 70-80. Patient A/Ox4. RN provided and encouraged small sips of juice and snack (cracker); continued to observe the patient for additional 15minutes. Patient back to baseline/normal. The patient left the observation site accompanied by his mother in stable condition.

95% Room air, 64 pulse, BP 112/72. Patient noticed to be less talkative. He had an almost syncopal episode. Patient never lost consciousness. Provided water. Patient recovered at vaccination table.

45 minutes after vaccine patient got dizzy and felt like he was about to faint. Felt nauseous and had to lie down. Then he felt very hot but no fever. Three hours later he felt better but said he would have crashed a car if he had been driving..

Patient is a 14 YO male with no previous medical history that received the Pfizer vaccine administered into his left Deltoid at 1406. He was sent to the observation area where the nurse noted him looking pale at 1410. His mother was present during this whole event. He was taken over to the medical tent for privacy and further evaluation. He was complaining of being light headed. He was given apple juice and crackers as he had stated that he had not eaten or drank anything all day. At 1430 his VS read BP 132/80, HR 145, O2 99%. At this time he was lying down and feet elevated, shaking, pale, and cold. Lung sounds clear. Nausea but not needing to throw up he was covered up to try and keep him warm. At 1440 his blood pressure was 130/82, HR 130, 02 98% At this time he was lying flat with feet elevated, shaking, pale, and cold. His lung sounds clear. Nausea, but not needing to throw up. At 1450 BP 138/74, HR 120-135, 02 98%. Symptoms had not changed at this point except he mentioned his ears felt cold, although they were warm to touch. He was laid on his left side at this point to try and lower his HR, which did come down to 115-125 when on his L side. We did not take a BP at this point as we wanted him to be on his back when taking them. At 1515 he was laid back on his back to see how his HR would do and how his BP was. BP 138/60, HR 135, 02 98%. Again symptoms were the same except he was complaining of colors being more ?dull?, and his teeth being numb. At this point his pupils were noted to be +8, equal and reactive. 1521 VS read 138/50, HR 135, 02 98%. Vision still dull and pupils unchanged from 1515. He was laid on his R side to see if this would lower his HR. It was brought down to 120-130 on his R side. We gave him a cookie to snack on to see if that would help with any of the paleness, shakiness, and light headiness. He was able to take a few small bites. Hands pale and washed, with +1 bilateral radial pulses. 1529 he was sat up slowly to try to get him to hydrate, as it was apparent he needed fluids onboard. His BP read 142/80, HR 124, 02 98%. He was still complaining of lightheadedness. After sitting up and drinking half a bottle of water and some cookie, he stated that colors were becoming more vibrant. He noticed my eyes were very blue, which previously he had stated they were grey. He stated his lightheadedness had improved and that he was feeling better. We had him sit up for a while and his color started to come back, although his L arm was still pale, of note this was the Vaccinated arm. At 1546 his VS read 150/82, HR 117, O2 99% while sitting. His only complaint was feeling like the air he was breathing ?was cold?. At 1607 he was stood up and when doing so his HR went to 140. After standing for about 4 minutes his BP read 120/82, HR 114, O2 98%. His pupils +6 , equal, round, and reactive. He was feeling way better and his color in his L hand was back equal to his right. He was slowly walked around a chair just to see how he would do with activity. His HR was anywhere from 94-120 when walking around the chair, he complained of no dizziness, or other symptoms. At this point he had finished 2 apple juices, crackers, a cookie, and a bottle of water for a total of 1.5 L of fluid. He was sat back down in the chair to rest and see how things went for a while. His HR was anywhere from 109-120 when in the chair, although he did appear very anxious through this whole process. When he was given a task it appeared that his HR was a few points lower. His final HR was 109, BP 130/82, O2 98%. Of note he did complain of his feet and fingers feeling tingly a few times during this process. Sometimes his feet would be tingly when elevated and then when the garbage can holding them up was readjusted it resolved. These were intermittent complaints. At this point it is 1630. He has responded to fluids and is stable to go home with his mom. It was recommended for him to increase his fluids tonight, and to eat something. He was sent with a bottle of water. It was also recommended to get a pulse ox machine from a pharmacy so that his HR could be monitored and if it does not come down under 100 by tomorrow to make an appointment with his MD to follow up. If any of these symptoms were to come back or if his mom had any concern at all it was recommended to take him to the ED for observation. It was also recommended to follow up with his primary even though he left stable. Throughout this process, I considered administering Epinephrine. The problem was his BP was not low and we could not get his HR to come down. I did not want to increase his HR any more than it already was unless I was absolutely necessary. Given that he responded to fluids, it was decided not to administer it. Given that it took him 2.5 hours to recover from this vaccine, I strongly recommend that he does not get his second dose. I consider this to be an adverse reaction, and his mom was in agreement.

Within minutes of getting 1st Pfizer vaccine he had a presyncopal episode. Stable vitals but felt dizzy/lightheaded, no LOC. Felt clammy and sweating on his forehead. Mom thought his color was draining from his face. Returned to baseline after 10 minutes of lying flat with feet elevated, cold water to drink, cold rag on face, and some fruits snacks.

Voiced complaints of dizziness and top of head headache. 1st vital signs - HR: 64 BP: 120/70 O2Sat%: 99 2nd set of vital signs - HR: 95 BP: 104/80

At 0950, client requested assistance from emergency medical technician (EMT). EMT?s responded and brought gravity-chair. Client reported feeling dizzy, nauseous, blurry vision, and hard of hearing. Client was assisted into gravity chair. EMT obtained vital signs at 0954: blood pressure 110/78, heart rate 67, oxygen saturation 97%, respiratory rate 18, alert & oriented x4, skin was warm and dry to touch. Registered Nurse (RN) was alerted and assessed client. Client?s father accompanied client and reported client had no allergies or chronic conditions. Client is not taking any medications. Client reported eating cereal for breakfast. RN provided vaccination education and was encouraged to follow-up with primary care provider to report the reaction and to complete the survey on VSAFE. Vital signs at 1002: blood pressure 108/78, heart rate 71, oxygen saturation 99%, respiratory rate 20. Client reported symptoms decreased to a ?3/10? from a ?10/10?. Vital signs at 1007: blood pressure 112/68, heart rate 80, oxygen saturation 99%, respiratory rate 16. Client reported symptoms mostly resolved. Vital signs at 1014: blood pressure 108/70, heart rate 81, oxygen saturation 97%, respiratory rate 18. Vital signs at 1024: blood pressure 102/66, heart rate 65, oxygen saturation 99%, respiratory rate 18. At 1030, client stood up from gravity chair unassisted and walked out of vaccine site unassisted with steady gait.

In the first few minutes, I had a very mild sharp, needle-like pain. The next morning, my arm hurt very badly. Throughout the day, I had moderate fatigue. Also, I had nausea, dizziness, chills, and a feverish feeling (all symptoms were wild). That night, I ate a light, protein-filled meal, 325 mg of Tylenol, laid down for a while, and went to sleep. When I woke up, I feel perfect and had no symptoms.

Systemic: Fainting / Unresponsive-Mild, Additional Details: Patient fainted 5 minutes after administration during 15 minute post-vaccine observation period. Per witnesses he slumped over to his left side and fell onto the floor to his left side loosing consciousness briefly. He was assessed by immunizing pharmacist and nurse practitioner and was found to have normal regular pulse & was pale in color. He regained consciousness shortly after a few seconds, EMS came over and assessed him later and was deemed ok to go home after 30mins.

Pt reported muffled hearing, nausea, dizziness BP 117/59; HR 86; RR 20. Pt reported resolved sx after rehydration. Pt advised to get rest & increase fluid intake, advised to f/u w/PCP if other delayed s/sx. Pt stable & d/c'd from vaccination site

59kg 14 yo M with NKDA and PMH of headaches had a syncopal episode after receiving first Covid19 vaccination in left deltoid at 1458 5/14/21 (lot # EW0179) . Patient described feeling light headed and does not recall losing consciousness. Fell face side down and had trauma of right knee and nose and above the left eye. Bruising on his nose. Reportedly hit the metal base of the curtain that separated the vaccination area from the observation area. EMS called for concern of facial fractures. BG 98/48 and HR 75 with O2 sats 99% post event. EKG negative. BG within normal limits. Patient did have sustained loss of consciousness. Followup BP 101/69 with HR 77. Event attributed to vasovagal reaction.

Patient was given vaccine and was asked to sit down for 15 minutes. Patient choose to stand near the out window. Patient started to feel dizzy and fell over. The patients dad was standing next to the patient and was able to catch the patient before lowering the patient to the ground. Noticed the patient was falling and called out to the pharmacist on duty. They ran outside to help the patient. Told to go get a bottle of water and instructed the patient to put their head between their knees. About 2 to 3 minutes passed and patient was able to move to the pharmacy waiting area with dad's help. After 15 minutes patient was able to leave the pharmacy under own power. no injuries reported. patient received the vaccine around 6:20pm and had reaction within 5 minutes

Patient passed out immediately following administration of the vaccine to the left deltoid. His head fell backwards and limp. The Rph administering the vaccine moved in front of the patient and gently moved his head into an upright position. The patient had mild muscle spasms and twitches while passed out but when he regained consciousness he immediately began talking saying he was ok. Father came into the consultation room at that point and Rph explained what had occurred. Patient looked a little pale and eyes were having a hard time staying open so with the help of his dad Rph helped him down to the ground in the corner of the room so he could have back and head support from the wall. Patient was given a glucose tablet and water, and later orange juice. Patient stayed in the consultation room for about 20 minutes before leaving on his own and saying he was feeling much almost back to normal.

Child received first dose of pfizer vaccine after a minute he turned pale and dizzy. He also said he could not see. His blood pressure was 94/71. He stayed for a half an hour and was feeling better after a half an hour. His color returned to normal and was able to function.

After receiving vaccine in the left deltoid, patient fell face down on to the floor. Patient was noted with what appeared to be two seizures lasting 10-15 seconds in duration. Patient received immediate attention, contacting EMS and recording vital signs. Blood pressure noted at 105/64, heart rate 55, 02 saturation noted at 99%. EMS arrived and provided immediate attention. Parental guardian (Mother) refused transportation to the emergency room. Mother was provided patient education in the risk of refusing medical care. Mother signed AMA form and stated she would take patient to the hospital.

Client received 1st COVID vaccine of Pfizer (Lot# EW0185 expiration 05/22/2021). Client has been sitting in vaccination chair for 5 minutes. RN provided supplemental nutrition and fluids. Client accepted fluids, but refused nutrition, states "I'm not hungry". At 1455, RN and PHN observed Client leaning forward with hand on his head. Client appeared to be pale. At 1456, RN and PHN assisted Client to gravity chair. Vital signs at this time are blood pressure 106/60, heart rate 78, oxygen saturation 97%. Client reports dizziness and blurred vision. Client continues to sit in gravity chair drinking fluids. Client is accompanied by his mother. Per Client's mother, there is no past medical history, no known allergies, but history of dizziness with vaccines. At 1459, Client reports no blurred vision. At 1503 vital signs are blood pressure 112/62, heart rate 77, oxygen saturation 98. Client reports no blurred vision, no dizziness, no headache, no nausea, no chest pain. RN educated Client and mother about ED precautions and common adverse effects of COVID vaccine. AT 1503, vital signs are blood pressure 110/62, heart rate 75, oxygen saturation 99%. Client reports no blurred vision, no dizziness, no shortness of breath, no nausea, no chest pain, skin tone returns to appropriate coloration to ethnicity. At 1507, Client is able to stand up without complaints. Client left vaccination facility at 1508 with a steady gait accompanied by mother

Dizziness, sweaty, blurred vision approx 2 mins after getting vaccine. Brought over to medical. Patient was alert, pale and diaphoretic. Patient was given water, crackers, alert and talkative, feeling much better. Patient sent home Past Medical Hx Had presyncope episodes with vaccine/blood draws in the past No recent illness and ate today

Patient vomited approx a few minutes after vaccine was given. Patient slowly fell onto the ground from the standing position. No injuries occured.

Systemic: Dizziness / Lightheadness-Severe, Systemic: Exhaustion / Lethargy-Severe, Systemic: Flushed / Sweating-Severe, Additional Details: patient developed sweating, pale lips, fatigue/lethargy, eyes closing and hard to remain concious. pharmacist called 911 and paramedics came, evaluated patient and blood pressure / heart rate normalized. patient able to sit down. after 30min post vaccination. patient and patient's father agreed to go home and refused ER service from paramedic.

Pt experienced headache, blurry vision, "muffled sounds," flushing/sweating & experienced dizziness. Juice was given, mom stated this happens for all shots, crackers given, pt lay down & rested. Provider cleared pt, pt stable & released from vaccination site

Sore throat, headache, body aches, fatigue, cough, dizziness, Fever and nausea. Still ongoing. Went to clinic to get COVID tested it was negative. Seen by a nurse practitioner, gave him meds for cough and nausea. Still hasn't resolved 7 days later.

PT STARTED SLOUCHING DOWN IN CHAIR, ASSISTED PT TO COT. BP 115/71, HR 81. PT STATES HE FEELS FINE NOW, PROVIDED WATER, SX RESOLVED, NO TRANSPORT.

Pt received first shot of Pfizer vaccine on 5/14 - 12:30 PM EST. On 05/17 around 7:45 AM, Pt passed out/fainted. He woke up few minutes prior to the event and this event started after he passed urine. As soon as he finished Urination, he felt nauseated, dizzy and then lost consciousness thereafter. He fell backwards as a result and hurt his back as well as back side of the head. During his unconsciousness his body was completely stiff and his eyes were rolled back. For few seconds then he became completely still (it felt he was not breathing). I gave him physical stimulus (slight slaps on the face) post which he started reviving. During this time we called 911. I gave him few sips of water forcibly and he started reviving thereafter. on 5/15 and 5/16 (post vaccination on 5/14) he was feeling tired and seemed bit dull. This was unusual as Pt is a competitive soccer player and highly active.

After 3 minutes of observation in a chair, he got up to move to the observation area in clinic and then fainted. He fell backward against a closed door and slid down. He quickly regained consciousness and his vitals were stable. within 10 minutes he was no longer dizzy or hot. He was able to sit up right without issues. Per dad, this is the same reaction he had to the flu vaccine in fall of 2020.

Patient felt dizzy and passed out, did not hit head, was lowered to the ground. All vital signs were normal throughout experience. Patient was transported with a wheelchair, ate some crackers and drank some juice. No concerns noted, patient left clinic site, self ambulated.

Pt received vaccine and felt light headed and nauseated. He vomited and felt warm. BP 108/68, hr 98. pulse ox 96-99% Placed cool rags at neck and head, and given water, within minutes pt felt better. Was accompanied by mother.

Pt c/o flushing/sweating and lightheaded/dizziness immediately after vaccination. H/o lightheadedness with flu vaccine in the past. Pt ate rice and eggs at 2pm today, no breakfast. Pt's symptoms resolved after drinking 500mL apple juice and laying down with knees bent. Advised taking 2nd dose supine. BP 109/69, HR 63, RR 18 -- > 108/68, 64, 16 -- > 112/79, 75, 16. Pt stable and released from the vaccination site.

Pt had a vasovagal episode right after the vaccine was administered. Pt c/o Flushing/sweating and dizziness. Initial BP 88/31 -- > 124/66, 76, 16 -- > 135/68, 67, 18. Pt recovered and was stable. Released from the vaccination site.

Patient (14 years old) fainted in the observation area receiving the first dose of the Pfizer vaccine. Patient fell from chair and had some scratches to right temporal region. Patient was accompanied by his mother. Patient was assisted to chair and appeared very pale and disoriented to time and place. EMS was called at 1138. Patient also stated his vision was blurred, reported nausea (emesis x1), and reported feeling very weak and ?really weird?. Patient also reported he was very sleepy and wanted to go to sleep. Patient was kept awake. Mother stated patient had history of ADHD and anxiety but no other past medical history. Paramedics arrived at 1145 and assessed patient. Blood glucose and VS were normal. Patient continued to report feeling unwell and wanted to go to the hospital but also wanted to ?stop for food?. Patient?s mother reported that patient was starting to sound like his normal self and that his color was returning. Patient?s mother agreed for the paramedics to transfer patient to ED for further evaluation. Patient left in ambulance at 1157.

1639 - directly after receiving covid vaccine pt became dizzy unable to walk to observation area. pt carried by EOA and father to cot. Pt responding slow but appropriate no dyspnea noted. EMS called. 1640 v/s 112/80, 99% o2 sat, 74 pulse, 16 respirations 1645 v/s 110/80, 80 pulse, 18 respirations, 99% 02 sat, dizziness getting better. EMS arrived pt stable refused to go to hospital. 1655 v/s 110/78, 78 pulse, 98% 02 sat, 18 respirations, asking for water sat up denies dizziness drank small amount of water. Denies dizziness. 1700 Pt walked to chair sat down no dizziness or c/o. 1715 Pt v/s 114/80, pulse 84, resp 16, 02 sat 98%, Pt discharged to home via PV with father. Instructed father good idea to follow up with PMD.

Pt felt light headed after witnessing brother's syncopal episode. BP 111/57, HR 79 -- > 115/71, 81. Pt felt better after resting. Pt stable and released from the vaccination site.

Systemic: Dizziness / Lightheadness-Severe, Systemic: Fainting / Unresponsive-Severe, Systemic: Seizure-Severe, Systemic: Vomiting-Severe, Additional Details: AFTER SHOT ADMINISTERED,PATIENT WAS WALKING TO WAITING AREA FOR OBSERVATION, WE FAINTED AND HAD 2 SEIZURES ABOUR 40 SECONDS LONG. NO PULS OR BREATHING. GAVE 10 CHEST COMPRESSIONS AND 2 BREATHS. EMERGENCY WORKER SHOPPING IN STORE HELPED ROLL TO RESCUE POSITION AFER FEEELING PULSE IN THE WRIST. HE SPIT UP A SMALL AMOUNT OF SPIT. HE SHORTLY CAME AORUND IN ABOUT ANOTHER MINUTE. NO EPINEPHRINE WAS ADMINISTERED.

Upon arrival a 14 y/o male with the chief complaint of general weakness and dizziness was found sitting upright in a POV. Vitals were as followed B/P 84/58, HR 68 O2 97%. A second ser of vitals were taken as followed B/P 120/75 HR 70 O2 99%. Lung sounds were clear and equal bilaterally, PERRL was present, PT skin was warm and dry. PT was COAx4. EMS personnel observed PT until he was stable and was returned to parental care.

Pt c/o dizziness, clammy UEs. BP 104/68, HR 88, RR 20 -- > 104/68, 90-105. Symptoms resolved, VSS, A&Ox3. Pt stable and released from the vaccination site.

PT STARTED TO FEEL NERVOUS. PT SAID HE FELT HE NEEDED SOME AIR SO HE WALKED OUTSIDE. MOTHER WITNESSED HIS FALL. PT FELL ON HIS ELBOWS, HANDS SHOOK FOR 2 TO 3 SECONDS. PT DID NOT RECALL FAINTING. REPORTED HAVING A HEADACHE IMMEDIATELY AFTER.

Patient reported feeling nauseous and light-headed post-vaccination. EMS evaluated patient and gave him water. 6/1/21: 11:37am - TC to parent. No answer. left message to call back. - RN 6/1/21: 12:22pm - Mom returned call. Child doing fine. No concerns or questions. - RN

Patient was administered the vaccine. After administration we stood talking to his mother who had questions. After about 2 minutes they turned to walk away and he lost consciousness and hit his head on a counter. I unfortunately did not witness him falling. He regained consciousness almost immediately and I told him he needed to stay laying down for a few minutes first. After a few minutes he said he felt fine and we moved him to a chair. Within a few minutes, he became ill again so we had him lay down again in the consultation room. He then laid there for about 15 minutes before we moved him to a chair. He stated that before he lost consciousness his heart was racing. His mother states he's never had any problems with vaccinations in the past. After sitting for the second time he said he felt fine except where he hit his head which was a little sore. We contacted the father later that night and he said that he too had a similar episode with a needle stick a few years prior. The patient felt better later night per his father.

Pt did well before and after vaccination. A few minutes later in the waiting area he started feeling ill and his mom had him sit down and she called 911. He complained of weakness, diminished sight and hearing and lightheadedness. He was very pale and sweaty. With his mom's permission I activated an ammonia inhalant near his nose. He said he felt better after the 2nd sniff. After a couple minutes he said his symptoms were greatly diminished and he was ok. The paramedics arrived and pt was able to walk to the ambulance for further evaluation. A paramedic came back and said the patient is feeling much better and declined to go to the hospital. I talked to the mom a couple hours later and she said he is all better.

The patient complained about feeling lightheaded, sweaty, and blurred vision. Spo2 99 HR 96, BP 124/98 T 95.1 The patient?s mother stated that he always gets that way when he receives a vaccine or donates blood. The clinical staff attended to him. He stated that he felt fine after about 5 minutes.

Patient fainted approximately 5 minutes after receiving the vaccine. He fell to the ground hitting his head on some shelving while falling. He was quickly revived by his father, but still felt dizzy and complained of blurred vision. Patient was given a bottle of water and comforted by his father and brother. When I checked on him about 20-30 minutes later, he said he felt fine. He looked a lot better too.

Patient(pt) received the first dose of Pfizer vaccine around 15:30pm. Pt had symptoms of weakness, dizziness, and lightheadedness right after the vaccination. Pt's father, coordinator, and I assisted pt to sit down on the closest chair. I checked pt's blood pressure (BP) immediately with reading of 71/33 mmHg, HR 50. 8 ounces of water was given to pt. Patient declined to lie down. put another chair to have pt leaned on. Pt was alert and oriented during the observation. About 10 minutes later, pt stated he felt a little better and started to drink water and eat candy bar, Pt finished 8 ounces of water and one candy bar. Second bottle of 8 ounces of water given to pt. BP checked again with reading of 113/73mmHg, HR 71, O2 Sat 98%. No nausea, vomiting, accessory muscle use, labor breathing, facial edema, edematous lips, or rash noticed. Pt was alert and oriented with clear speech. Pt finished the second bottle of 8 ounces water. 20 minutes after receiving Pfizer covid vaccine, pt was able to ambulate without assistance. Re-checked pt's BP with reading of 95/59 mmHg, HR 70, O2 Sat 98%. Pt's father denied that pt has any type of allergy to medications or vaccines but stated patient is always afraid of needles, and he fainted last time with his mother after receiving vaccine. Patient stated he wanted to go home. ER precautions given to pt's father. Side effects and allergy reactions of Pfizer covid vaccine discussed with pt's father and educated pt's father to give pt Tylenol for fever, body aches, or arm pain as needed, increase water intake, have enough rest, and avoid NSAIDs use. Pt's father verbalized understanding. Patient ambulated with steady gait to their car. Pt and his father left at 15:53pm.

He was given the shot standing up. After the shot he pulled down his sleeve started walking to the doorway hit the wall and fell to the floor. He was immediately wiped down with a wet cloth and a fan was placed beside him. I was wiping his face and stomach with the cloth.

Patient received 2nd dose of vaccine. right after the recommended 15 minutes wait, as patient was about to leave, he stood up and fell over. I responded by checking his pulse. He recovered while on the floor and said he felt dizzy and blanked out. Patient also said that he did not have breakfast before coming for the vaccine. I made the patient to wait for another 30 minutes to observe him. He left feeling better.

Rapid onset of warmth feeling followed quickly by "visual changes" and dizziness. Brief syncope sitting without falling occurred with rapid recovery. Little to no diaphoresis or nausea. Low normal range BP's recorded in the range of 90-100 sys and 50-60 diastolic. Pulse 70-80. 02 sat -98% No orthostasis. Typical vasovagal reaction. Discussed with father & patient.

On the 4th - woke up weak; light headed; and a lot of discomfort - his arm really hurt. General body achiness. After a little while, he got up and was able to take some Tylenol and attend his video school day with frequent breaks. In afternoon, he started to feel worse. 102. 6 fever - we called Tele-doc appt. His throat was sore and he had some chest pain. The doctor recommended that we go to ER to have him fully evaluated. Pain and fever got better with the Tylenol. He took another couple of dose. Next morning, he had a lot better but continued to have a low fever 100.2 - on and off that day. His lymph nodes under his left arm were quite swollen and painful. The fever did not continue yesterday but the swelling has and the pain has. He was very light-headed and brain fogged on Sunday and Monday. Seems to be better today - the lymph nodes have gone down a lot but are still swollen today. He is in process of recovering.

PATIENT EXPERIENCED DIZZINESS AFTER 2ND COVID SHOT. MOTHER WAS PRESENT. HAD HIM PUT HIS HEAD BETWEEN HIS LEGS, MONITORED BLOOD PRESSURE, ICE PACK ON THE BACK OF HIS NECK. HE STARTED TO FEEL BETTER. SINCE HE WAS IMPROVING WE LET HIM STAY IN THE CLINIC ROOM WITH A TECH MONITORING UNTIL HE COULD STAND BACK UP. GAVE HIM A JUICE AS WELL FOR WHEN HE WAS FEELING BETTER. PATIENT FULLY RECOVERED AND WAS FINE PRIOR TO LEAVING THE STORE.

2 days post vaccination patient developed hives--parent treated with Benadryl, 3 days post vaccination--patient's finger began swelling, worsening symptoms over following 4 days included increased difficulty walking (started with a limp), diarrhea, extreme pain with inability to walk Patient went to ER--admitted and given IV Toradol

The Epidemics arrived, he did feel better then, can sit on chair for them to check up. His vital signs came up normal, he was observed for 15 more minutes before allow to go home

A 14-year-old male patient received BNT162B2 (PFIZER-BIONTECH COVID-19 VACCINE), at same age, dose 1 via an unspecified route of administration, administered in Arm Left on 19May2021 (Batch/Lot Number: ew0177) as 1ST DOSE, SINGLE DOSE for covid-19 immunisation. Medical history included asthma. Concomitant medication included fluticasone propionate (FLOVENT); montelukast sodium (SINGULAIR); and cetirizine hydrochloride (ZYRTEC) and METHYLPHINATE. The patient previously took peanut, tree nut, coconut. The patient experienced shortness of breath, hives on mouth, throat swelling, and dizziness all on 19May2021 14:00 with outcome of recovered. The patient did not receive any other vaccines within 4 weeks prior to the COVID vaccine. Treatment antihistamine was received for the adverse event. Prior to vaccination, patient was not diagnosed with COVID-19. Since the vaccination, patient has not been tested for COVID-19.

Dizzy, low blood pressure, tingling in hands and feet. Patient was placed in supine position, legs elevated. Oxygen was administered. BP slowly returned to normal, he was slowly transitioned from supine to sitting then standing and was able to walk out of office on his own. Event lasted about 1 hour.

Arm hurt, could barely move it. Then he got the chills and a fever, and really tired. Hours later, he said his tongue started to hurt. I tried to log this on the daily check in but by the time I clicked on the link it said it expired - 12 hours after receiving - why wouldn't they give you 24 hours?

After waiting 15 minutes after the administration of the Pfizer vaccine, the patient and his mom left the pharmacy. According to the mother, the patient told her in the parking lot that he was having dizziness and a little bit of shortness of breath. So they came back to the pharmacy. The mother also stated that he had asthma when he was younger but that she suspected he grew out of it. He had no other symptoms of a possible allergic reaction. I gave him 10ml of Benadryl (25mg) as a precaution. They stayed 25 more minutes after Benadryl as a precaution, as the mother also contacted his physician for further directions. After that 25 minutes, the patient said he was feeling fine, and that they will be going to get something to eat.

14yo male that was admitted to our hospital's cardiology service prior to after an episode of acute onset tachycardia (possible atrial tachycardia), dizziness, headache at school. He had his first covid vaccine 8 days prior to the event at school. He has a coincidental diagnosis of an anomalous left coronary off the right aortic valve cusps, but his coronary course is unusual where he would be very unlikely to be at risk for sudden cardiac arrest. He had an ECHO in the ED that showed a mild left ventricular dysfunction (EF 45%). He underwent a cardiac MRI on 5/28/2021 was done that was normal without myocardial edema or scarring. His ejection fraction on that study (24 hours later) was 54%. He was discharged home and had a stress test the following week (6/1/2021) that showed blocked PACs at peak exercise (a very usual finding for an otherwise healthy kid) and possible slow atrial tachycardia at baseline. His laboratory evaluation including CRP and ESR was normal. 72 hour Holter monitor showed blocked PACs and no supraventricular or ventricular tachycardia. Mother reports that he is "back to baseline" May 31st. Of note, his father was covid positive with headache in October 2020. The patient also had a headache for 1 week without fever. It was presumed that he had covid as well, but was not tested per PCP recommendation.

Within 5 minutes of vaccination he was in the waiting room and became very dizzy, sweating and was having trouble breathing. I reported to the closest worker and she called in another person, assuming one was a nurse and one was not. She had him look into her eyes, which was difficult, then she had him out his head between his knees for a minute and he was then able to come out of it. Still a bit dizzy, they moved him to a room with a bed where he could lie down and they put a cold, wet paper towel in his forehead. He rested there for an additional 10 minutes. They had him get up and walk before they allowed him to leave. He had not eaten anything since the night before and he has severe ?needle anxiety ? which both could have contributed to the reaction. He has not had any issues since, except for a sore arm.

During monitoring period post Pfizer COVID-19 vaccine, pt experienced lightheadedness, feeling faint, sweating. 2 minutes post vaccine, BP was 108/70, P-64, O2 100% RA. Patient was easily aroused, stated he hadn't eaten or had hydration for > 12 hours and "needles make him nervous." Crackers and orange juice provided. After extended observation patient stated he felt better and symptoms were relieved.

1258 Called to lobby. Pt pale, diaphoretic, and having tremors of extremities. Per mom he does this with vaccines. She did not say anything thinking it was psychological and if she did not mention it he would be OK. C/O dizzy. Transferred by parents to wheelchair then to treatment room. Two person standby assist to treatment table and lying in supine position. Verbalizes had not eaten and feels better lying down. 1300 VS 117-67 HR 76, Resp 14, O2 sat 97% room air. 1314 Skin color improving. Skin dry. Assisted to sitting position. 1315 VS 116/71 HR 71 R 14 O2 sat 98% room air. Dad assisted with ambulating to lobby waiting area. Instructed to sit for 20 min and will reassess. If dizziness returns to not stand up and let parent know. Verbalizes understanding

06/08/21 2nd dose of Pfzier.Vaccination administered @ 10:44. According to client arm fasciculation ? started one minute after vaccine. Nausea 2 minutes after. Complaints of dizziness. Respirations 24 at 10:59. Respirations 28 and irregular at 11:04. Tightness in throat @11:10. Tongue not swollen. Breath sounds unobstructed(report given via school nurse), RN, PHN 06/09/2021 Mom reports child had fever and lump in throat the evening of event. The next morning child vommited x1. RN, PHN 06/11/2021 Mom reports no new side effects since the morning of 06/09/21 and the child is feeling fine. RN, PHN

About 3-5 min after vaccination patient felt unwell, hot, dizzy, feeling of passing out, had his head down between hands. Pharmacist and patients mom got patient down on the ground flat with legs elevated on chair, a cold pack and water. Pharmacist did not witness loss of conscientious. Patient was nervous about getting vaccine. We called 911, emergency services came, did a work up. It appeared to be a vagal response, possibly from fear of needles. When emergency services got patient up he seemed a little nauseous. They took him to hospital.

Patient began to feel light headed and sweaty after 5 minutes of receiving the vaccine. Pharmacist laid patient down where he started to feel better and regained color in face. After a 2-3 minutes patient was sat up again to read blood pressure where it read 90s/80s and in 5 minutes 80s/60s where patient started to feel light headed again; during this time paramedics were called and came within 5-10 minutes where they assessed patient.

5 minutes after injection: reported feeling hot + dizzy, then passed out momentarily. Recovered to normal within seconds. Approx 90 minutes after injection: rapid onset of extreme headache (outward pressure), difficulty breathing, chills, shaking, extreme fatigue, feeling of racing heart, mildly impaired vision, mildly impaired thought. These symptoms continued in waves over the subsequent 2-3 hours. Between waves, reported feeling fine.

On the 5th day patient woke with nausea and extreme exhaustion and weakness in legs. After a few hours nausea subsided but limb weakness persisted. Patient had a hard time walking unassisted. Visited Dr office in early afternoon where neurological assessment occurred. Patient limb strength was normal while seated. All other tests passed. Patient tested negative for covid in pcr exam that day. Symptoms increasingly improved over next few days and symptoms are non existent 1 week following second vaccination on June 5th

a 14 year old male with no significant past medical history who was in his usual state of health until he after his 2nd dose of Pfizer COVID-19 vaccination on Friday afternoon 6/11. On Saturday 6/12 he had headache, fever in the AM and had poor oral intake during the day and in the afternoon had developed mid sternal chest pain that Described as thumping and wrapped around his chest under his arms. He felt that it was worse with inspiration and continued to worsen despite tylenol. He went to a medical center where his workup on Saturday evening 6/12 was normal (including EKG and blood work). He also had some dizziness at that time but no syncope. He was changed from as needed tylenol to as needed motrin and instructed to take famotidine. On Sunday his chest pain persisted but was stable and he continued on motrin. On Monday 6/14 he had chest pain that felt like pounding this AM, headache, body aches, and pain in his bilateral arms, mouth and throat. He had some nausea but no vomiting or diarrhea. He represented to a medical center where his troponin was 0.25 ng/ml (normal 0-0.03 ng/ml) pro-BNP 324 (normal 0-125 pg/ml); ESR 9; CK 359; CK-MB 20.3; WBC 3.9; Hgb 14.1; plts 181. And he was noted to have ST elevations on EKG. He received toradol at 2:30pm with some improvement in his pain. His pain level has been variable 5-10/10; currently reporting that his chest pain is resolved. He has had no shortness of breath, palpitations, sick contacts, URI symptoms including cough, congestion, runny nose; no travel.

fever to 101 F on 6/17/2021 at 8am which persisted. Chest pain started 6/18/2021 at 3am and has been constant. sharp chest pain lower chest midline, worse with laying, better with sitting or walking. Shortness of breath with laying also. Also has palpitations, light headedness, dizziness, nausea, feels clammy. Chest pain worse on 6/19/2021 at 1:30 am and came to the ED for further evaluation and treatment. Just got admitted and full evaluation in progress.

Patient has a history of Vasovagal Syncope which was unknown at time of vaccination. Patient felt like he was passing out, he lied down immediately and started to feel better.

Systemic: Nausea-Medium, Systemic: Seizure-Severe, Additional Details: Patient received vaccine and within 2 minutes complained of nausea& lightheadedness to his mother then passed out. Upon putting him on the ground he began to seize. Paramedics called & came & took vitals. All within normal limits. Patient was alert & talking & stated he felt fine. Upon being sat up in a chair his eyes rolled back & he began to seize again, more violently. He was then placed on strecher & taking to ER.

Patient was given doe1 of the Pfizer COVID-19 vaccination. Shortly upon standing, the patient experienced an episode of syncope and fell to the floor hitting his head on the counter. The staff immediately called 911 and the pharmacist provided supportive measures until First responders arrived. First responders examined the patient and cleared him to go home with his parents.

Pt reported lightheadedness after injection. Pt moved to floor then relocated to different medical floor 1. VSS. Water and crackers administered. Pt reported lack of symptoms but after standing symptoms returned and pt positive for orthostatic hypotension. After multiple attempts at standing and no improvement over an hour's time, CVC clinician recommended further medical follow up. Mother decided to take pt to Emergency room/urgent care. Pt released to parent's car via wheelchair.

About 18 minutes after receiving the vaccine, the patient collapsed on the elevator and was unresponsive for a couple minutes. He reported feeling dizzy and nauseated, and he appeared pale. His blood pressure was 103/69, and his heart rate was 52. Of note, he had not eaten breakfast, nor had any water to drink on the morning of the event. He was given apple juice and some crackers, and his symptoms resolved within about 20 minutes.

We went to pharmacy for second COVID-10 P-fizer dose. Around 12:15 PM on 06/23/2021 my son Vaccinated and with in one minute my son have start problem. he feel Dizziness. I hold my son very quickly and close his eyes. During dizziness he also complained his vision is low and with in 15-20 second he complained he can see anything with his open eyes. He can listen my word but he can not see. I check his body and it is cold. Nurse check his body temp and it is normal. After 10 minute rest he is ok . He can see everything and he feel little laziness. This is a full story. He drink water after his dizziness gone. I am still watchin him in case if anything. Thanks.

Patient experienced near syncopal episode following his vaccine: Within 3-4 minutes following administration of Pfizer Covid-19 vaccine patient reported feeling lightheaded and dizzy. Patient was found to be pale and slightly diaphoretic. Patient was helped to supine position with legs elevated and rested while symptoms fully subsided. Pt was able to answer questions about himself and his symptoms throughout duration of event and did not lose consciousness. Patient was monitored closely for an additional 45 minutes, VS monitored and provided with a light snack and water. After full resolution of symptoms patient was escorted to his car with parent. Mom was provided with education regarding the event and signs and symptoms to monitor.

Patient was given vaccine with no initial side effects and was asked to wait for 15 minutes before leaving facility. During that wait time patient started to develop blurred vision, and the pharmacist was alerted that the patient fell on the way to the bathroom. Patient was then guided into the immunization room when he briefly lost consciousness. While patient was seated, pharmacist assessed for anaphylaxis reaction and contacted emergency services. Blood pressure was taken and noted to be elevated, other symptoms included blurred vision and sweating. Patient denied itching, swelling, or trouble breathing. Patient did not receive epi-pen and was assessed by emergency services. Patient recovered shortly after emergency services arrived, no further services were needed

14yo M initially presented to the ED with headache, fever, myalgia, abdominal pain, NBNB emesis, unsteady gait and soft/garbled speech, 6 days after returning from a recent trip to Mexico and 4 days after receiving 2nd dose of Covid-19 vaccine. In the ED patient had multiple tonic clonic seizures, total episode lasting about 5 minutes requiring ativan and keppra. Pt was admitted to PICU for management, found to have continued seizures on EEG resolved with fosphenytoin

Client 5 minutes after receiving Pfizer 1 st dose vaccine client felt lightheaded. Client laid down to floor supine and was awake and alert. Client face was flushed and client appeared anxious--HR 80bpm. Client's extremities cool and client reported tongue was swelling and bilateral arms were tingling. Epi pen 0.3 mg IM administered in left thigh and 911 called. Client remained alert until paramedics arrived--breathing labored and hands clenched down. Mother at side

Had seizure. Fell in shower, started shaking uncontrollably, had difficulty breathing. He had a seizure for the first time in his life 6 days after first dose (already reported). This one was his second seizure ever.

The patient received the vaccine and 4-5 minutes later he fell out of his chair, facedown. He appeared to be convulsing. He was pale, diaphoretic, and disoriented. His blood pressure was 76/36 and his pulse was 84. Radial pulse was 2+. EMT was called. His EKG appeared to have a slight elevation in his inferior leads. He and his guardian deny any significant history. Upon discharge with EMT, the patient was alert and tolerating liquids well. The patient's guardian states that he has had a similar reaction prior when he was the age of 6. She states that he also felt slightly lightheaded from the first Pfizer vaccination.

My son experience after the vaccine around 03:40pm, he felt dizzy, started to black out and passed out. I gave him fluids and called his PCP. I made him eat since he did not eat earlier that day. He states he felt off, I gave him some Tylenol medication. He also had a headaches and started with diarrhea. The next day he stayed in bed, continued with flushed bright red cheeks, diarrhea and the headaches. The day after that he started to feel bit better, I think his symptoms lasted between 36-48 hours. His glucose level was 90.

After receiving COVID-19 Pfizer vaccine at noon, patient returned home and began mowing lawn. Shortly after, while mowing, felt lightheaded then passed out - was unconscious for < one minute. Had water that morning and food, but may not have had adequate water? Has history of being nervous with vaccines but had never passed out before

Patient was feeling dizzy when walking in the parking lot to the car with his father about 15 minutes after his first Pfizer Covid19 vaccine. He sat down for a few seconds and felt ok but when he went to stand up to continue walking he passed out. They did not re-enter the store. He regained consciousness and went home. His father was with him and is observing him at home.

1 week after receiving vaccine #2, PT complained of fatigue and. Started getting lightheaded with standing and walking. Day 11 after the vaccine he complained of chest pain, so I took in to the ED at Hospital. His EKG, CXR, tropinin, CK,BNP were normal but he was orthostatic by heart rate. He followed up with Dr. and he recommended push fluids, more salt/Gatorade. He has continued to have orthostatic tachycardia and when he walks household distances his HR goes up to 120-140. He is lightheaded with minimal exertion. Climbing one flight of stairs makes him short of breath. He has an appointment with Dr. tomorrow, 7-5 and I will request more labs and cardiology referral. PT was in good shape and played basketball before the vaccine.

Patient received dose 1 of Pfizer Covid vaccine on 7/7/21 at 1115AM. After two or three minutes, patient lost consciousness and fell off the chair in the waiting area and recovered a few seconds later. Patient felt very hot and vision became blurry. No airway restriction observed. 911 was called and paramedics transported patient (completely awake and coherent) to local hospital for further evaluation

Patient felt as though he was going to pass out. Became diaphoretic, pale, nausea. Patient was given juice, cold pack applied to neck and forehead. Placed on his back. He felt better about 5 mins after treated.

After receiving his first dose of Pfizer Covid vaccine was standing while mother was asking a few questions and as they were about to walk over to the observation area, he passed out and fell. He hit his head on the corner of a metal plate. Pt may have been "out" for ~30 sec and came to immediately and was A+Ox3. All his vital signs were stable (BP 118/66, HR 86, O2 100). Mother was present and the ambulance was called at 10:23. Mother denies past history of fainting with IZs, denies remarkable mhx. Paramedics arrived by 10:28 and per mom's request was taken to ED.

Felt faint, hand numbness/tingling, turned pale in color, nausea 15 minutes after receiving 1st dose of Pfizer COVID vaccine. Patient put head between legs and a cold compress on neck. Took blood pressure 20 minutes, 30 minutes, and 35 minutes after vaccination. Blood pressure range 93-102/62-68. Pulse range 76-104.

Approximately 5 minutes after receiving vaccine, patient got lightheaded and started to pass out. He did lose consciousness. He was helped to the floor and we elevated his feet. We also gave him a cool compress for his forehead. After about 3 minutes, he moved to a chair.

Prior to administration of the vaccine, I discussed with mom and patient about previous immunizations and the YES response to previously fainting or felt dizzy post immunization. Mom proceeded to state that he has a strong vaso-vagal response to needles/immunizations and he has fainted multiple times during and after immunizations. The patient appeared calm, so I did some breathing exercises with him and when I determined that he was calm, I administered the vaccine. Patient was still sitting upright, I asked him to continue to breathe slowly and deeply. I asked if he would like a water, and so I went to grab it from the pharmacy lab. Patient then stated he was dizzy and was going to pass out. He did not pass out, and I asked him if he would like to lay down. I put down a towel for him and then he started to throw up. I got a cold washcloth for his neck. He started sweating profusely (essentially a marked cholinergic response). I got him to lay down after some more vomiting . He stated that his vision was blurry, but he was laying down calmly. after 25 minutes of reclining, he stated he was starting to feel a bit better. I offered a 25mg diphenhydramine tablet to help with residual nausea. He stated he was better and go shop with his family. They did not return to the pharmacy, so as of my last interaction, he was fully recovered from the event.

Pt began complaining of dizziness after receiving Pfizer vaccine, while sitting in recovery area. Pt was pale and clammy to touch. Pt's father stated that Pt did not eat anything today prior to coming in for vaccination. Pt was evaluated by staff, and given a bottle of water and a granola bar. Pt ate and drank what was provided, stated he felt better, and left clinic in care of his parents without further incident.

A 14-year-old male patient received BNT162B2 (PFIZER-BIONTECH COVID-19 VACCINE, Solution for injection, Lot Number: EW0187), via an unspecified route of administration on 29Jun2021 at 09:40 (at the age of 14-year- old), as dose 1, single in left arm for COVID-19 immunisation at pharmacy. The patient's medical history was reported as none and concomitant medications were not reported. The patient did not receive any other vaccine on the same date of the Pfizer vaccine and no other vaccines were taken within 4 weeks of COVID vaccine. On 29Jun2021 at 09:40, right after the first dose of vaccine, the patient experienced allergic reaction and had swollen throat and tongue/his throat was swollen, difficulty breathing/he couldn't breathe/his throat started closing up, hives all over his body, and heart palpitations; all for a duration of 1 day 14 hours and 20 minutes. On 30Jun2021, 14 hours and 20 minutes after the administration of first dose of vaccine, the patient was dizzy, for a duration of 2 days. Later, the patient felt better. Treatment received included Epi Pen. Reporter stated that the pharmacist called the ambulance and took him to the Emergency Room for Pediatrics where he received Benadryl and something else for his breathing. Adverse event resulted in visit to emergency room on 29Jun2021 and physician office for allergic reaction. The clinical outcome of the event dizzy was resolved on 02Jul2021 and clinical outcome of all other events was resolved on 01Jul2021. Follow-up attempts are completed. No further information is expected.

Patient received the vaccine in left arm by Pharmacist. Patient stated a phobia of needles. Shot administration was normal. Patient walked out of the vaccine room and told his mom he felt light headed and dizzy. She had him sit down to which he fainted and was unresponsive for 45 seconds or so. Mother was present and got pharmacy attention. The reaction was at 10:39 am and by 10:43am patient was talking and answering questions but was still very pale. EMS 911 was called and patient was given water. EMS arrived on seen and patients blood pressure was 83/49 and oxygen was 97. EMS decided to transport patient to emergency services.

after vaccine was administered, patient was told to wait in the store for 15 minutes. Within 2 minutes, patient told their mom, he felt "woozy". Patient dropped to his knees. Pharmacist was alerted by immunizing technician. Pharmacist went to the lobby to assist. Patient was alert but stated he did not feel good. Pharmacist helped patient to a chair. within another minute, patient stiffened and would not answer to his name. 911 was called. ambulance arrived within 5 minutes. Patient was alert, but pale, said felt sweaty, and dizzy. paramedics took blood pressure and place the patient on the gurney. blood pressure was normal. paramedics stated they would run a few tests in the ambulance, and mom agreed to going to hospital after. patient left the store with paramedics.

Patient was vaccinated with his first dose of Covid Pfizer vaccine and approximately 10-11 minutes later his mother stated that he no longer felt like sitting. He went to stand by her and 3-4 minutes later patient had a syncopal episode, fell back, and hit his head on a wall. The patient had recovered consciousness by the time staff reached him. Our staff came to his aid and had him sit up, assessed for any signs of bleeding, which there were not, and checked vitals (results below). Patient had no complaints of headache, no dizziness, was alert and oriented (was able to repeat name, DOB, address, and where he was), no nausea, no neck or back pain, just mentioned that he was standing then everything went dark. He mentioned that his left arm was tingling and sore but nothing he would be alarmed about. At the time, we suggested we monitor him an additional 15 minutes and for him to inform us if he felt any additional side effects. At approximately 3:55pm, patient mentioned that his hands felt cold, but otherwise he had feeling and no numbness. Patient mom was concerned and agreed to call emergency services. Ambulance arrived and took patient to hospital at approximately 4:09pm.

After 10 minutes of receiving the Pfizer Covid 19 vaccine the patient felt light headed and laid down on the chair. Patient sat back up and was alert and oriented. Knew his name, the day, and where he was. There was no signs of anaphylaxis. His pulse was faint. After 3 unsuccessful attempts at getting a blood pressure 5 minutes apart I called 911. The paramedics came and the first BP was 107/80 and pulse was 60. After they took a finger stick glucose reading His BP dropped to 80/44. They waited 5 minutes and rechecked . His BP came back up to 100/60 and his pulse was WNL. They told the patients mom it was ok to go home and monitor the patient. If it reoccur or PT is not feeling well call 911 or go to the ER

A 14-years-old male patient received first dose of BNT162B2 (PFIZER-BIONTECH COVID-19 VACCINE, solution for injection; Batch/Lot Number: EW0179; Expiration Date: 31Aug2021; NDC number and UPC number: unknown) via intramuscular on 15May2021 (at the age of 14-years-old) in left arm (shoulder) as a DOSE 1, SINGLE for covid-19 immunization and second dose of BNT162B2 (PFIZER-BIONTECH COVID-19 VACCINE, solution for injection; Batch/Lot Number: EW0182; Expiration Date: 31Aug2021; NDC number and UPC number: unknown) via intramuscular on 05Jun2021 (at the age of 14-years-old) in left arm (shoulder) as DOSE 2, SINGLE for covid-19 immunization. Medical history included papilloma viral infection from 06May2021 to 06May2021 and bowel movement irregularity from an unknown date and unknown if ongoing. The report was not related to a study or programme. Prior vaccinations (within 4 weeks) included HPV (human papilloma virus) vaccine on 06May2021. Concomitant medication included macrogol 3350 (MIRALAX) as he had trouble going to the bathroom because he didn't had control and had trouble with bowel movement, he started when he left the hospital, so he can get back to his bowel, but he stopped using it couple of days ago as he started to go to the bathroom on his own again. The patient received both doses of Pfizer covid vaccine. He had a serious reaction to the vaccine, he couldn't go to the bathroom on his own, he had no control from his waist down, he needed a catheter as he was not able to pee or poop. He was admitted to the emergency room on 16Jun2021 and spent 3 days in the ICU because he could not walk and move, spent 3 days in out the down rated version, total 6 days in the hospital. He was diagnosed with Guillain-Barre syndrome and he was in the hospital on his birthday in Jun2021 and was doing physical therapy to walk and run again. His mother was trying to deal with the covid compensation because they had work. She and her husband were alone in the hospital. She reported that their medical bills were going to be crazy because they had to go by ambulance from one hospital to other because they were afraid of Guillain-Barre syndrome. The patient didn't have any movement in his legs all the way up to his heart. He was not able to play any sport because he can't run. The patient was discharged from the hospital and was on physical therapy (water therapy). The patient underwent lab tests and procedures which included two MRI, spinal test, and blood work with unknown results in the emergency room as they weren't sure what was going on with him. The patient was treated with IVIG and blood thinner everyday two times a day. The outcome of all the events were unknown. Information on Lot/Batch number was available. Additional information has been requested. Follow-up attempts are completed. No further information is expected.

a few seconds after client reported to RN feeling dizzy at 1715. RN set up anti-gravity chair and client was able to transfer without assistance. RN then informed PHN client was not feeling well and prior to vaccine had stated he had a headache. Mother had told RN client's headache was minor and to still administer vaccine. PHN arrived at 1717, client alert and oriented x4, sitting in Fowler's position, pale. Vitals at 1717: pulse 95, oxygen sat 99%. Per client new onset of nausea and stomachache, RN gave client emesis bag and water. Blood pressure at 1719: 118/76, pulse 75. Per client's Mother, client has medical history of migraines, no current medications and no known allergies. Per client had ate earlier today, but spent all day out in the sun. Client stated his migraines are often accompanied with nausea and vomiting. Client informed he would be kept 30 mins in observation. Vitals at 1724: blood pressure 120/72, pulse 75, oxygen sat 99%. Per client dizziness same, headache slightly improving 5/10. At 1726 client reported needing to use restroom. EMT guided client to restroom. Client able to transfer self again back to anti-gravity chair. Vitals at 1735: blood pressure 125/76, pulse 78, oxygen 99%. Client reported feeling "slightly dizzy", headache improving and nausea had resolved. Client stayed with EMT remainder of observation. Vitals at 1741: blood pressure 120/78, pulse 70, oxygen sat 99%. Per client headache 3/10, dizziness and nausea resolved. At 1746 client reported mild headache. Vitals at 1810: blood pressure 120/78, pulse 72, oxygen sat 99%. Per client headache same. EMT educated client and Mother to follow up with medical provider and gave ER precautions. Client left facility with steady gait at 1812

PT c/o headache, pain 6/10, and dizziness. vitals temp 98.5, pulse 66, BP 125/66, resp 17, pO2 98, glucose 94 seen earler in day for headache but no c/o headache on assessment,, apparent change in mental state, health care provider decided to transfer child, transported via EMS to Hospital- dx bradycardia, treated and realeased back to EIS

Presented with bilateral facial weakness on 7/11. Prescribed 3 day course of steroids (7/14-7/16) without improvement. Presented again on 7/18 with worsening facial weakness in addition to bilateral upper and lower extremity weakness and difficulty walking. Admitted to hospital on 7/19. On the second day of his hospitalization he was no longer ambulatory. Diagnosed with likely AIDP based on CSF studies (albuminocytologic dissociation, negative infectious work-up). Treated with 3-day course of IVIG (7/21-7/23), after which he showed significant improvement in his upper/lower extremity and facial strength and ambulation. He is being discharged with ongoing outpatient physical therapy.

Patient was anxious and scared "worked up" prior to vaccine per mom. The patient received Pfizer vaccine at 3pm and at the vaccine table he was verbalizing he was scared,nervous and his hands were cold and clammy, he felt dizzy. He sat at the vaccinator table and we gave him water and crackers, VS 2:57 BP 129/70, HR 78, O2sat 1005. VS 2:58pm 129/80, HR 78, O2sat 98%. VS 3:01pm BP 125/69, HR 70, O2sat 100%.Patient taken to observation area,stated feeling better, walked without feeling dizzy and continued monitoring VS 3:03pm. BP 123/64, HR 68, O2sat 100%. 3:06pm eating cookie VS BP 122/69, HR 64, O2sat 100%. 3:12pm VS BP122/64, HR 60 and sat 100%. Patient stating feeling better, not lightheaded or dizzy. VS 3:16pm BP 124/62, HR 58 and O2sat 100%. Patient left at 3:20 with mom "feeling good".

PATIENT RECEIVED PFIZER COVID VACCINE AND WENT TO WAITING AREA. PATIENT COMPLAINED THAT HE FELT FAINT. PATIENT DIAPHORETIC AND SHAKING. CLOSED EYES. APPEARED TO LOSE CONSCIOUSNESS FOR ABOU A MINUTE. GAVE PATIENT COLD WATER. PATIENT REVIVED QUICKLY. MOVED HIM FROM CHAIR TO WAITING AREA TO LAY FLAT. HE WANTED TO RISE TO HIS FEET WITHIN MINUTES. PATIENT'S MOTHER IS PHYSICIAN AND WAS ON PHONE WITH FATHER ACCOMPANYING HIM.

Pt was at 2nd dose scheduling & had syncopal episode. Assisted fall to the floor by mother. EMS C-spine negative; VS stable (BP 126/78; HR 87; RR 18). Pt felt better after juice & water.

Patient feel dizzy, light headedness, vision changes, observe peel, muscles relax. Did not loose consciousness during brief event, was alert and oriented there after. Escorted by nurse and family member to observation area. observed for 30mins.

Patient felt dizzy, hot, nauseous and subsequently fainted approximately five to ten minutes after vaccine administration. He regained consciousness within a few seconds. They had him lay on the floor with feet elevated. Dizziness and lightheadedness resolved. He remained under observation for 30 more minutes.

Patient brought to medical tent after collapsing after he got vaccinated. Nursing reports he has been unconscious for 2 minutes after standing up and was caught by nurse. Pt was sweaty which got better when supine. Pt felt weak, headache, dizziness and pale. no difficulty swallowing. attempted to sit up and dizziness worsened. Pt's symptoms did not improve so EMS initiated.

Patient become light headed, lost consciousness and struck head. He rest under observation, ice pack and drunk juice. He recovered and discharged home Pulse : 67 SPO2 : 99% BP: 110/58

Systemic: Fainting / Unresponsive-Severe, Additional Details: while patient was waiting out in the waiting area for their 15 minute monitoring session the patient passed out and fell to the floor. The patient did not injure himself with falling to the floor and woke up immediately after he passed out. We immediately put ice packs on him and had him drink a bottle of water while sitting on the floor and made him stay in the waiting area until his color came back in his face and felt fine to go home. He was extremely nervous about getting the vaccine.

Pt had just received vaccine and stated he "wasnt feeling well", and laid down on the floor. Pt assisted from floor to wheel chair and vitals obtained : BP 150/80, HR 118, RR 20, SpO2 97% RA. PT complained of headache, nausea, and not being able to walk. Pt father advised EMS that pt "often get's emotional and works himself up like this" and did not show concern to Pt state. Pt stated he wanted to go to the hospital, Pt father insisted that he take Pt to hospital via POV. Pt father signed refusal of TP form, and PT care TF to Pt father.

PATIENT WAS GIVEN THE COVID-19 PFIZER VACCINE @11:18AM. THEY WERE ASKED TO STAY 15 MINUTES AT THE WAITING AREA AND AROUND 11:21AM, HE FAINTED. I RAN TO THE PATIENT, HE WAS ALREADY CONSCIOUS BUT STILL CALLED 911. PARAMEDICS CAME 15 MINUTES LATER, HE WAS ALMOST RELEASED TO GO HOME BUT UPON STANDING UP, HE FAINTED AGAIN. HE WAS TAKEN TO HOSPITAL AND WAS DISCHARGED SAME DAY.

Patient lost consciousness and fell on the floor 5 mins after receiving the shot. He had no response to any physical contact. His pulse was weak and pupils were dilated. One dose of 0.3mg Epipen was then given to the left thigh of patient. He regained consciousness in about 5 mins after the Epipen shot. His pulse and pupils turned back to normal as well. He was able to stand and answer questions to the EMS team when they arrived to the scene in another 5 mins. Patient was then taken to the ambulance.

12:15pm pt. was seen sitting in wheelchair with EMT and dad at his side. EMT stated pt. c/o dizziness, nausea and blurred vision after taking Pfizer COVID-19 vaccine. At 12:13 vital signs taken by EMT staff. HR 74, BP 104/70, O2Sat 99%. 12:20pm pt. denies any pain or discomfort. 12:30pm pt. left vaccine site to go home via wheel chair. EMT and dat at his dice with no signs of acute distress.

client signaled to EMT for assistance. Client was a 15 year-old male found sitting upright, alert and tracking and stating he was dizzy. Client had a chief complaint of dizziness. Client stated that he began to feel dizzy approximately "2-3 minutes" after his vaccination. Client denied falling and denied any injury. Client denied any allergies or past medical history. Assessment revealed no swelling, discoloration, hives, shortness of breath or other signs of anaphylaxis. Client stated that he felt dizzy but denied any pain throughout his body. Client appeared mildly pale but was alert and oriented to person, place, date and event. Client's airway, breathing and circulation were intact. Vital signs: pulse 78, O2 99%, Respirations 12, Blood pressure 118/82, Eyes equal and reactive to light, lung sounds clear bilaterally. Temperature 97.8. Client and his mother consented to an additional 30 minute observation. Client had a granola bar and juice while waiting and his color pallor subsided. After lying down for five minutes, client stated that he felt better. Remaining vitals were: 0945 - 99% O2, pulse 76, respirations 18, blood pressure 118/78, 1000 - 116/78, Pulse 70, respirations 16 99% O2, 1010 - 118/78, Pulse 76, Respirations 16, 99% O2. Client's mother was provided with all relevant information and instructed to seek medical care or the emergency room if signs of a severe allergic reaction occur later. No medications were administered.

Post vaccine pt became dizzy and lightheaded. Pt transferred to med obs via wc. Pt reports immediately feeling better after sitting down on litter. Vitals obtained and monitored. Pt had no complaint of chest pain. Pt had no complaint of SOB. Pt released at 14:10. Vitals as follows: 1359 SITTING BP 116/67 HR 62 RR 16 SpO2 98% 1410 STANDING BP 119/79 HR 62 RR 16 SpO2 98%

Shortly after exiting the pharmacy, the patient's dad came back to the pharmacy propping up his son. Stated his son got light-headed and almost fainted. The pharmacist had the patient come into the vaccine area, sat him down, and took his blood pressure (106/80). His breathing was monitored and when it checked out, the pharmacist gave a single dose of diphenhydramine 25mg. The patient drank 2 x 16oz of water. Paramedics were called as a precaution. The patient was able to walk out on his own. The pharmacist made a follow up call at 2PM and the father acknowledged that his son is doing fine.

Pt c/o dizziness. Pt told to rest laying down and was given juice/crackers. Pt return to baseline. A&O x4. BP 99/64, HR 84, RR 18. Pt stable and released from the vaccination site.

Client received the 1st COVID vaccine of Pfizer (Lot#EW0185 expiration 05/22/21) At 4:28PM client reported dizziness, lightheaded, headache, no blurred vision, no chest pain, no shortness of breath. Client has been waiting for 20 minutes in the observation area. at 4:33PM RN responded to event. Vital signs are blood pressure 100/62, hear rate 73, oxygen saturation 99%. Client reports slight headache towards the frontal lobe. Per client and client's father, there is a history of asthma and no known allergies. At 4:38PM EMT obtain vital signs that are blood pressure 110/60, heart rate 74, oxygen saturation 99%. Client reported feeling weak, no dizziness, and no lightheaded. Provided supplemental nutrition and fluids to client. Client accepted. RN educated client and father about ED precautions and common adverse effects of COVID vaccine. At 4:47PM vital signs are blood pressure 100/60, heart rate 82, oxygen saturation 99%. Client reports not feeling symptoms. All symptoms had resolved. Client has steady gate and is waiting for other siblings to complete observation time.

Patient complained about feeling lightheaded and nauseous. EMT called. Vitals BP 76/53 HR 63 O2 sat 99% and blood sugar 112. Per patient's mom, that is his normal vitals. Patient given water and laid down in ambulance. Patient refused EMT accompany to hospital and states he is feeling better.

Patient received vaccine at 5:30pm and started to feel dizzy. Patient was given water, juice and snack. Patient after 5 minutes felt tingling in the arms and lips. EMS was called to take patient to the hospital.

Patient had vasovagal event at Vaccine clinic. LOC < 10 seconds, Reported feeling dizzy, nauseous, and lightheaded. Patient was laid down on cot, VSS. Laid for 10 min, sat with legs dangling for 10 minutes. Given juice for sugar boost and water. Patient's mother present. States he has had episodes like this in the past. Patient is a vegetarian. Denies HA, difficulty breathing, SOB, CP. 1:08pm VS: BP: 84/62 O2: 98% HR: 52 - laying PERRLA, HRR, no vomiting. 1:13pm VS: 90/59, O2:98%, HR:54 Discharged at 1:30pm in stable condition in care of mother. Return/ED precautions given. -

Pt was given vaccine and began telling his brother he felt lightheaded. Pt's brother began to inform his mother. Pt then slumped in chair and began to fall to the floor. Mother tried to brace pt's fall. Pt still hit the floor and lightly hit his head. LPN's stated pt had a 5 second episode of convulsions that appeared to be a seizure. Pt did lose LOC for about 15 seconds and came to. Pt did not realize what had happened. Pt was placed on a cot. Pt stated he felt warm and lightheaded. Pt's VS were taken at 1640 - BP 136/70, HR 61, RR 18, 99% O2. EMS were called and in route. Pt began feeling better. Another set of VS were taken at 1645 - BP 127/76, HR 80, RR 16, 99% O2. Pt had ice packs placed on his neck and forehead. Pt sat up on cot after another 5 minutes and another set of VS were taken at 1650 - BP 123/82, HR 89, RR 16, 99% O2. Pt stated he was feeling better but still had a minor headache. EMS arrived and advised pt to be checked out. Mother agreed to take her son to the hospital herself not via EMS. Refusal form was signed by mother from EMS. Pt, mother, and brother left to go get checked out.

Was sitting in observation room, felt dizzy and put his hands on his head, then passed out. Hit head on carpeted surface. LOC less than 10 seconds.

We sat him down in the chair and had him relax. I called EMS shortly after and they arrived quickly. They worked him up and concluded that he was having a panic attack related to being give the vaccine. Patient states that he wasnt anxious about it previous to administration nor did he seem anxious as well. He was calmed down by the EMS personnel and then was able to walk out of the store

patient had the vaccination and waited for 15 minutes, then when he was walking out the store then he felt dizzy and fainted for seconds, Dad was there to catch him when he was falling. he came back to conscious as I was there, we called 911 and paramedics came and checked on pt. then he went home with parents after he was done with the checkup with the paramedics.

Patient felt nauseous and lightheaded several minutes after receiving vaccine while sitting in chair in observation area. Was given ice pack, water, juice and snack. Taken to health suite via WC and placed on cot, supine with LEs elevated to rest for several minutes. VSS. Symptoms resolved after several minutes and patient able to go home with father.

Patient received first dose of Pfizer vaccine. Instantly became nauseous and Faint/pale. Assisted him to the floor elevated legs. No LOC. Pulse was 80, BP 138/80

near syncope, got dizzy and pale BP: 107/58 @ 9:23am HR 66 provided water. within a few minutes patient felt fine. BP rechecked @9:36am was 117/68 HR: 90 patient recovered and ready to go home.

PT REPORTED FEELING ANXIUS AND DIZZY, ALSO REPORTS BLURRY VISOIN PT REPORTS FEELING BETTER AFTER ABOUT 5 MIN, HR 80, 96% O2, PT REPORTS SMILAR REACTION TO PREVIOUS VACCIN DID NOT TASKE ANY MEDICATION TODAY PT WAS ENCOURAGE TO LIE FLAT AND TO HYDRATE PT LEFT SITE WITH PARENT AFTER 15 MIN WAS RESOLVED SYMPTOMS

Client reported feeling, "lightheaded" approximately 10 minutes following the vaccine administration. Patient was immediately placed in the anti-gravity chair, and given food and water. The patient denied history of fainting but reported similar experiences with previous vaccines. The lightheadedness was relieved following the consumption of food, water, and placing in the patient in a semi-Fowler's position.

About 3 min after giving vaccine, pt was seen potentially having a seizure. Unconscious for about 5 seconds. Came to. Hot, sweating, dizzy. Dizziness continued for approximately 45 minutes.

Patient experienced as 15- 20 second syncopy spell when he sat up frpm a sitting position after receiving the covid vaccine initial vitals 102/60 pulse 40 after regaining conciousness BP was 111/60 pulse at 60 patient prior to fainting appeared diapharatic, pale and cool to the touch there were no signs of llegeric reaction including flushing of the face lack of hives or angiodema no wheezibg, no strider or labored breathing patient complained of dizzness upopn standind equal to 30 mins.. EMS was notrified when arrived patient was up and alert without dicculties EMS recommended patient be released to his gaurdian BP at release 112/57 pulse 62 father advised to call physician if any concerns arise or go to nearest hospital in an emergent situation

After injection, patient stood up and we noticed he was falling. We caught him, laid him on the floor and called 911. His blood pressure was 75/33 and heart rate was 57. Emergency crew took him to the hospital. I called his mom in he evening and she said he was fine, he got nervous during vaccination

Syncopal episode with Loss of Consciousness for several seconds . Radial pulse was weak Heart rate was 48 / min and BP 85/60 . Heart rate and BP improved 112/80 Struck back of head during fall

(Vasovagal Response) Fainted when standing up after getting vaccine. Hit shoulder on table. C/o of mild pain to shoulder, but no additional complaints. Laid down and no additional treatment needed.

dizziness, lightheaded, black out, cold/clammy skin, profuse sweating, ringing in the ears 911 was called and responded to by both fire and medical paramedics vitals were taken and patient was observed for an additional 20 minutes before leaving when he had returned to a stable condition

one minute after injection, pt started to fall forward in the chair. father caught him and I was immediately called into the immunization room. pt was laid onto the floor and his legs were elevated onto a chair. father stated pt had never had a reaction to a vaccine before. no health conditions. 911 was called..... but then parent decided he didn't want them. pt states his vision started to go black before he briefly passed out... probable vasovagal syncope. we waited for 911 to come since they were already alerted. pt sat in chair but then started to get lightheaded again, so he returned to a position where his feet were elevated in the chair. EMS arrived and monitored him. father and EMS made decision to return pt to home.

Systemic: Dizziness / Lightheadness-Severe, Systemic: Seizure-Mild, Additional Details: patient suddenly fell down, had a slight seizure last around 5 sec and regains concious. He feels fine but pharmacy still called 911 and was escort to receive emergery/urgent services.

Pt walking and c/o unable to see, pale, diaphoretic and lightheaded. W/C to med eval station for further evaluation. Vision returned after 2 minutes, Gatorade and water offered. EMS evaluated with VS 90/60, HR 70, Sat's 98%. Lying on cot with parents at side. No further visual impairment. Able to see without any blurring or peripheral impairment. EMS continue to monitor, taking fluids, no s/s neuro deficit present with assessment. Able to stand with no c/o's dizziness. BP 110/70 HR 88 Sat 98 RA. Continued recover, parents decline any hospital transportation. Ambulatory discharge with parents and contact pediatrician for any further symptoms. Reviewed with parents well hydrated today and s/s anaphylactic reaction. Pt to be vaccinated 2nd dose in med eval area when he returns for 2nd dose, parents verbalized understanding.

Pt experienced a lightheaded feeling and dizziness after receiving his first dose of the Pfizer vaccine. Pt was then assisted in ambulating to the treatment area where he was placed in a supine position on a the medical cot. Pt never completely lost consciousness. Vitals were obtained every 5 minutes while in recovery. After approximately 2 minutes, the Pt began to feel better and was able to sit up with no negative symptoms. Pt remained in observation for another 15 minutes and was then released with no complications or incidents.

Person complained of light headiness, dizziness and blurred vision after receiving the first dose of the Pfizer vaccine. EMS was called and treated the individual on scene and was released on their own.

Patient received the COVID vaccine. Patient stood up to allow the next person to sit for vaccination. Within a minute, client loses consciousness and falls, hitting his head on the wooden floor. Client was evaluated by our emergency medical technician, vital signs within normal limits. Client presented with palor, coloring resolved prior to leaving the site. Client was transferred to the cot. Client alert and oriented on all spheres but states he is tired and wants to sleep. Medic unit called for further evaluation.

Pt reported to briefly lose consciousness and stated that his vision changed. Pt did not fall completely from chair. Pt was gently lowered to the ground. Pt was alert and oriented x 3 and vision restored while lying on the ground. Patient was given water but then when he sat up, he soon became unconscious again and again was gently lowered to the floor. Regurgitated/ spit out the water. After initial confusion, was alert and oriented again.

Pt dad thinks pt has vasovagal syncope. Pt passed out immediately after getting vaccinated and vomited when regained consciousness. Pt turned white in the face and fell forward in chair when he fainted and after vomiting pt continued to go in and out of consciousness. Took about 10-15 minutes of placing feet on another chair and raising hands over head but pt color returned and he felt much better.

Patient exhibited signs and symptoms of syncope. Felt dizzy, clammy skin, nauseous. Had him lie down for 15 minutes with legs elevated and used an ice pack on the neck to help improve blood flow. Patient recovered.

The next day experienced migraine and arm soreness for approximately 24 hours. Today, 5/25/21 patient woke at 4am to dizziness that has continued throughout the morning, 8am. Patient is alert and virtual learning.

he received the first dose of vaccine and fell to the floor almost immediately, paramedics responded, BP was low but no other issues, he was released without further medical attention

dizzy, headache, blurred vision, vomited after vaccination. (at 03:52 pm) outcome: recovered after drinking water and eating crackers and discharged to home.

Syncope, Vomiting, flushing/sweating. BP 122/47, HR 51, RR 16 -- > 125/71, 54, 16. Pt fell to the floor but recovered after resting. Pt stable and released from the vaccination site.

Client received the 1st dose of Pfizer COVID vaccine lot #EW0185 and expiration date 6/24/21 at 5:52PM. Per vaccinator, reports that client was saying, "Wow that was aniclimactic" after getting vaccine. Per vaccinator, she notices within 1-2 minutes client got quiet and then reported "I am not feeling well." Client received vaccine in left deltoid. RN requested PHN 1 to get gravity chair. PHN grabbed gravity chair and asked RN to get BP cuff and pulse ox. PHN arrive to assess client and found client alert and oriented x 4 in chair at 5:53PM. Client was already drinking water given by vaccinator when PHN arrived. Client reports he was experiencing "dizziness and slight shortness of breath." PHN ask client if he thought he was steady enough to switch to gravity chair. Client stood up slowly and sat in the gravity chair. RN 1 asked client if his throat felt tight-client responds-"No" and coached client to take deep, slow breaths. At 5:55PM vitals are: blood pressure: 130/90, pulse: 55 and oxygen: 98%. At 5:57pm client reports no longer feeling shortness of breath. Client is speaking in full sentences and able to answer all PHN questions about chest pain and headache and client reports having neither. At 6pm vitals: blood pressure:125/90, pulse:75, and oxygen:100%. PHN instruct RN to get Lead PHN or Co-Lead. Client reports he has slight dizziness and feels lightheaded and reports that earlier he had blurred vision but now is seeing clearly. At 6:03PM vitals are: blood pressure:160/100, client appears pale and heart rate:140, and oxygen: 100%. Lead PHN arrives to take over assessment and PHN starts calling 9-1-1 at 6:04PM as blood pressure is repeated by co-lead :180/90, oxygen100% and pulse 129 and upon auscultation hears clear lungs and heart palpitations. Client is then reports shortness of breath and dizziness, no headache and no blurred vision. Parent reports that client has a history of asthma but rarely needs inhaler so she does not have it on her and that client recently had been on antibiotics for Gastrointestinal history and a bad cough (not COVID)-all symptoms had been resolved before receiving vaccine. At 6:07pm Vitals: blood pressure: 170/90, pulse:100%; no change in symptoms. At 6:08pm vitals: blood pressure:170/90, pulse: 94, and oxygen: 100%, client reports experiencing tightness in chest and unable to complete breath. Client reports no chest pain. Vitals taken again by RN due to new symptom, blood pressure: 122/70, pulse:93 and oxygen: 100%. RN auscultate again and hears palpitations have slowed and lungs clear. At 6:10PM, vitals are blood pressure:124/74 and pulse: 96. Client reports feeling "shaky, tightness on chest" and could not take full breaths. Paramedics assumed care at 6:12pm. Client and parent left on stretcher with paramedics to be transported to Hospital. Tell security that mother is leaving blue pathfinder in parking lot. Security assures he will not tow vehicle.

Temporary blindness (lasted about 30 secs to a min), pale skin, sweating, feeling cold, dizziness. Gave water, apple juice and Gatorade to patient and called 911. EMT came in to take he's BP, blood glucose level and both were normal. Stayed at pharmacy for about 45 minutes and patient got better but was still feeling cold. They've decided not to go to hospital but followed up with dad around at 6:30 - 7 pm ish, and confirmed he is doing okay.

Systemic: Chills-Mild, Systemic: Confusion-Mild, Systemic: Dizziness / Lightheadness-Mild, Systemic: Fainting / Unresponsive-Mild, Systemic: Flushed / Sweating-Mild, Systemic: Visual Changes/Disturbances-Mild, Additional Details: PATIENT RECIEVED VACCINE AT 2P. Sat in waiting area for about 10 minutes. Our observer noticed that the patient was sliding in his seat and looked dizzy. Patient starting to flutter his eyes and lost consciousness for about 5 secs. He complained about blurry vision, feeling hot, sweating, but with chills. Patient was given water. After he drank the bottle of water patient felt fine. The ambulance was called and arrived, patient reported he felt fine. Patient did not eat/nor drink water today.

Hypotensive. Dizzy and sweating. pulse 52 hr @ 12:46, BP 110/40. 12:51 BP 90/50. 12:54 BP 96/60. 1:00pm BP 108/68.

My son is very lightheaded and dizzy. Can?t get out of bed for 3 days. Nauseas and feels week. This comes in waves. Went to ER 2x and they can?t find anything causing it

Six days (evening of 5/20) after the first dose of the COVID vaccine (Pfizer), Patient started to experience severe dizziness and nausea. Since that evening he has experienced the following: - Significant increase in heart rate when transitioning from laying down to sitting and/or standing. - Severe dizziness - Excessive fatigue - Headaches - Gastrointestinal issues (constant nausea. Nausea increases in severity when standing. Diarrhea.) - Muscle pain in his upper arms and upper legs The above symptoms have continued 24/7 since their onset.

Patient and his Mother were in the vaccination room. appox 30 sec to 1 min following Vaccination patient stated he feels like he is going to pass out and before his mother or administering pharmacist could respond the patient fainted/ lost consciousness for approximately 30seconds

1022am- Observed patient abruptly stand up and fall to floor, landing face down. Turned patient over on back with neck support. Patients legs elevated. Patient was unresponsive to voice, performed mild sternal rub and became responsive immediately after stimulation. Pulse strong and regular, respirations WNL - VS BP 126/82, Sp02 99% RA, P122, Temp 98.2 Infrared, AAO to person, place and time. Observed ~1" laceration above right eye, treated with bandage and bleeding controlled. EMS and parent called to scene. EMS arrival at 1033, I gave patient report and they assumed care and assessed patient. VS @1038- BP 120/68, SpO2 99% RA, P101, R17 AAOx4, Patient c/o headache. EMS sat patient up on floor, then to chair. EMS had discussion with parent and it was agreed that parent would take patient to Urgent Care for treatment and parent signed off for no EMS transport. Patient walked out with parent and EMS on own power to vehicle and left approximately 11:00 AM.

Within 30 seconds of administration, patient felt dizzy and lightheaded. He never lost consciousness, but he did stumble and almost faint. Mother caught him - she says he has a history of dizziness after vaccines and is nervous about needles. He was very hot and sweaty, and he did vomit once. He was given water and an ice pack. Epinephrine was on-hand, but not needed. After resting for 15-20 minutes, he felt better and mother took him home.

Reports ate a few peanut butter pretzels today. Reported lightheadedness that resolved within about 1 minute, followed by seeing spots for about 2 minutes resolved, momentary tightness on inspiration that resolved, and ringing in ears for about 3 minutes resolved. Later reported nausea on ambulation post vaccination to seat in observation area that resolved upon sitting down. Also, reported has history of similar s/sx with flu vaccination. Denies chest pain and other s/sx. Nurses responded with emergency cart. 01:47pm LUE 96/70 pulse regular R20, lungs clear, lips noted to be dusky/purplish. Benadryl 50mg PO x1 administered. Lips color reassessed and after few minutes returned to usual pink color. Reports nausea, lightheadedness, visual spots, and ringing in ears resolved, all s/sx resolved, stated "I feel normal". Counseled follow up with pediatrician, urgent care or emergency department for any return s/sx, Mom verbalized understanding. Observed for 30 minutes. Ambulated with steady gait off floor with his parent and sibling.

it looked like the blood vessels or veins in the bend of elbow, inside of the elbow that looked like possibly a blood vessels that burst there.; feels sick; his body didn't feel right and he felt that way again yesterday when he felt so bad.; numbness and tingling right arm; tingling/numbness and tingling right arm; rash on the right arm; he has pain and a rash on his right arm; feeling like passing out; dizziness/lightheadedness; nausea; chest tightness/ chest was so tight and pulling.; pain in the arm at the injection site; headache; This is a spontaneous report from a contactable consumer (patient's mother). A 15-year-old male patient received the first dose of BNT162B2 (PFIZER-BIONTECH COVID-19 VACCINE), dose 1 via an unspecified route of administration, administered in Arm Left on 14May2021 11:15 (Batch/Lot Number: EW0173; Expiration Date: Aug2021) as 1ST DOSE, SINGLE for covid-19 immunisation (Age at vaccination: 15 years). Medical history included asthma, allergy, acne, pain, Gastroparesis, eczema, red blotches /bright red dots on his whole body and anaphylaxis. Concomitant medications included cetirizine (CETIRIZINE) taken for allergy; beclometasone dipropionate (QVAR) taken for asthma; mometasone furoate (MOMETASONE FUROATE) taken for allergy; isotretinoin (ZENATANE) taken for acne; paracetamol (TYLENOL) taken for pain; sodium chloride (AYR SALINE, both nasal spray and nasal gel) taken for allergy; herbal extract nos, homeopathics nos (SIMILASAN) taken for allergy, all are ongoing. The patient previously took advil [ibuprofen] and experienced anaphylactic reaction (when he was about 4 years old in about 2009 or 2010.). The reporter who is mother of patient calling regarding the Pfizer Covid 19 vaccine. States she wanted to call and report some side effects from the vaccine for her son. Reports he received the first dose of the Pfizer Covid 19 vaccine 14May2021 at 11 15 AM in the upper left arm. He stayed extra minutes after the shot because he had anaphylaxis before and had red blotches before in the past. Reports after receiving the first dose of the Pfizer Covid 19 vaccine, when they got to the car he said his chest was so tight and pulling. That lasted 1 to 2 minutes only on that day (on 14May2021, 12:00). Since then and even on the very first day he has had dizziness; lightheadedness; and felt like passing out (on 14May2021, 12:00). Adds he has been having to go to the nurse's office for headaches for the last two days (occurred on 14May2021). The reporter states her son has been very disappointed and mad at her for making him have the vaccine. Nausea - she adds her son also had some nausea (on 14May2021, 12:00). Right arm - he has pain and a rash on his right arm, the opposite arm from the shot, with numbness and tingling right arm (occurred on 17May2021); tingling. When he went to the nurse someone in that office mentioned that it looked like the blood vessels or veins in the bend of elbow, inside of the elbow that looked like possibly a blood vessels that burst there on an unspecified date. Pain in the arm at the injection site (occurred on 14May2021, 12:00) from the first day that is improved. The patient's body didn't feel right - when they started walking to the car his body didn't feel right and he felt that way again yesterday when he felt so bad. The first day and yesterday he felt his body didn't feel right (occurred on 17May2021, 11:45). Dizziness - is mostly when standing up and is lasting a lesser amount of time and getting better. Feels the sensation of like passing out with the dizziness on standing. Headaches - he has every day but yesterday was terribly bad and today were the worst. The reporter mentions that the patient stated the he feels sick on an unspecified date. Outcome of the events loss of consciousness, dizziness and vaccination site pain was recovering. Outcome of the event chest tightness was recovered on 14May2021 while outcome of the events numbness and tingling right arm was recovered on 18May2021. Outcome of the events pain in arm, vascular injury and malaise was unknown. Outcome of the remaining events was not recovered.

This information was reported to this writer. Immediately after receiving the second dose of the Pfizer vaccine, the patient reported a "weird sensation" in his chest. The patient became pale, and reported nausea and lightheadedness. According to the vaccinator it seemed as though the patient went into a syncopal episode. A nurse attempted to do a sternal rub to wake the patient up but was unsuccessful. The patient was then laid onto the ground and woke up; attempted to sit patient up and then went into another syncopal like episode. EMTs were notified and were on their way to attend to the patient. Patient's feet were elevated. No history of vaccine reaction. Patient had eaten and had fluids 2 hours prior to the vaccine. Patient was sat up again with assistance but immediately felt lightheaded and had another syncopal like episode. He was again placed in a supine position with feet elevated. Immediately reported improvement of symptoms. Patient had several sips of juice and was placed on a stretcher and taken to a back room. Patient had mom and sister at his side. As the patient remained in supine position, he denied nausea and lightheadedness; no chest pain, no dizziness. Patient was alert and oriented. Remained pale in color according to his father who just arrived. Per the advice of the EMTs, patient was transferred to hospital per ambulance.

At time of injection, patient seemed tense and told his dad he was dizzy. Vaccinator noticed "clenching of the muscle" and thought they saw a hematoma bump when the muscle clenched. Gave patient apple juice and let patient rest with father and observed for 15 minutes. Patient and family (father and sibling) left pharmacy by walking on their own with no issues

Patient c/o dizziness, no SOB, no LOC, alert & verbal, no n/v, no pain/discomfort. Patient assisted to gurney in supine position with HOB elevated. Pt given water/ vital signs monitored BP 106/91 pulse-81, RR - 17, ~2 sat - 98%, RA 2:50pm - patient verbalized relief from dizziness, no SOB, no headache, no n/v, alert & verbal, lip color returned back to normal, no LOC. Vital signs BP 108/77, pulse - 91, RR - 17, 02 Sat 98% RA

Systemic: Fainting / Unresponsive-Mild, Additional Details: Patient fainted, but was still awake and responsive, just lightheaded. Patient left on own accord after interaction with paramedics

"1:26pm- 15y/o male reported he was feeling dizziness and was brought to the observation room. Pt vomited a small amount. 1:28pm - Pt was transferred to bed. VSS and WNL. BP 111/71, HR 83, RR 24. Pt had no prior hx of dizziness, vomiting after vaccination. Pt was checked by MD. No Resp distress, allergic rxn to vaccination was observed. Pt drank 16oz water and reported feeling better. 1:39pm - Second vitals 111/70, HR 84, RR 20. Pt walked around the room w/ PA and was communicating w/ father & RN. 1:41pm Pt stable and released from the vaccination site with father."

Severe headache 10 out of 10 started the following day with vertigo. During this time he developed an episode of sudden onset double vision and vomiting which resolved after approximately 10 minutes. The headache and the vertigo lasted for several days.

Situation: Patient, 15M had an appointment for 2nd dose Pfizer (Lot#: EW0187) today. Shortly after receiving the vaccine, the patient became dizzy and pale. Background: Patient is allergic to antibiotics. Pt has history of appendectomy, absent sz and childhood epilepsy. He Was brought in to room via wheelchair after receiving second vaccine, Pfizer. Mother with pt. Pt already had 2 juice boxes. Pt complaining of increased dizziness and starting to get pale Assessment ( RN): @0753 68/48 (54) 113 97.1F 100%RA. Placed on supine position. Pt felt better when he was laid flat. @0800 90/51 (64) 73 18 100%RA @0803 88/61 (70) 58 18 100%RA @0807 91/62 (68) 55 18 100%RA @0815 104/58 (76) 55 18 99%RA, slightly dizziness @0820 109/55 (82) 57 16 99%RA, increased HOB to semi fowlers @0825 104/66 (80) 57 18 99% RA @0830 100/65 (80) 58 16 100%RA ? Pt intermittently gets dizzy and pale. Encourage increase intake. Pt drank another juice and cup of water. @0835 108/77 (89) 61 18 100%RA ? Increased HOB. Denies dizziness with position change. Able to drink another juice box and ate crackers. Conversing well with staff and mother. @0850 ? sat on the edge of the bed with nurse assist. Tol well. Denies dizziness or difficulty @0857 ? slight dizziness when standing up. Assisted back on sitting on the edge of the bed. VS117/77 (84) 66 16 99%RA @0904 ? 111/72 (85) 16 90 95%RA @0909 ? Assisted patient on standing up. Able to stand well. Denies any dizziness with position change. Able to take a couple of steps around. Pt stated that he feels back to baseline. Pt and mother felt confident that pt can go home. Assisted pt to wheelchair. Instructions discussed with pt and mother to hydrate and rest. Both verbalized understanding @0913 ? Wheeled patient out to car with mother. Recommendation: Increase hydration and rest. Patient and mom verbalized understanding. Patient able to go home.

AS MY CERTIFIED IMMUNIZING TECH WAS ADMINISTERING THE VACCINE, THE PATIENT'S HEAD LOLLED TO THE SIDE (SYNCOPE). IMMUNIZING TECH STUCK HER ARM OUT TO KEEP PT FROM FALLING FROM CHAIR AND HE REGAINED CONSCIOUSNESS ALMOST IMMEDIATELY (LESS THAN 1 SECOND). PT SAID HE FELT DIZZY AND WE HAD HIM STAY IN CHAIR WITH HIS HEAD BETWEEN HIS KNEES, TAKING DEEP BREATHS. HE WAS WITH HIS MOTHER, WHO SAID HE HAD DONE THIS BEFORE AND SHE DID NOT SEEM ALARMED.

Patient describes feeling dizzy, having visual trouble immediately receiving the vaccine, then slid onto the floor from the chair he was sitting on. Presents with mother who denies him hitting his head. LOC lasted 1-2 minutes before he was able to be roused. Upon regaining consciousness, he was pale, diaphoretic, weak. Reports feeling nervous with "needles". Observed for 30 minutes with mother. Escorted to car afterward and permitted to go home. Advised to seek medical attention if symptoms persist.

Systemic: Dizziness / Lightheadness-Severe, Systemic: Fainting / Unresponsive-Severe, Systemic: Shakiness-Severe, Additional Details: PATIENT FAINTED ROUGHLY 30 SECONDS AFTER RECEIVING COVID VACCINATION

Patient has severe anxiety when getting vaccines per Dad. Patient did eat before vaccine at around 10am. He stayed conscious the entire time, but had trouble standing and said he felt like he could not breathe.

Pt was given his 2nd dose of Pfizer vaccine at pharmacy on 6/3/21 at 1:20pm. Minutes after the vaccine pt developed headache, stiffness of neck, dizziness, fatigue and SOB. Epipen was brought out but never administered, pt was there for about 45 min. Was sent home and took Advil 400mg. Was playing badminton with lots of running 30-45 min prior to vaccine. No other vaccines given in the past year. Since the vaccine, pt has had fever up to 102. Continued headache, neck stiffness. 1st night 6/3/21- bad headache, throbbing headache all around head but worse back right side and then "spikes" on left side (like someone was hitting him with an axe) and fell asleep d/t sever pain, trouble breathing in the next few days 2nd night 6/4/21- neck pain and stiffness was really bad, he looked pale and almost disoriented. Had fever. Slow to respond. Would get rush of bad headache if he moved his neck too fast. Afraid he would fall down if he moved too fast. 3rd night 6/5/21 - (called 911) chest pain, difficulty breathing, vomited stomach acid, pain radiated down chest to the left, pain only when breathing in and went away the next AM 6/7/21 Chest still feels sore on Left side. Biggest issue right now is headache.

Patient reports feeling light headed with headache. denied shortness of breath, dyspnea, hives or itchiness. Patient educated to lay flat with feet up. Also encouraged to drink water to stay hydrated. after few minutes symptoms subsided.

Client received the 2nd dose of the COVID vaccine (PFIZER Lot # ER8736 expiration date 07/01/2021) at 1812. At 1819, client raised his hand as EMT was walking by and said, ?I?m feeling dizzy.? PHN overheard and proceeded to assess the client. Client stated that he also had a ?headache.? EMT and PHN transferred the client into the zero gravity chair at 1820. Client?s skin was pale. Client was alert and oriented times four. PHN offered the client a juice which he accepted and drank. After drinking the juice client stated, ?the dizziness has gone down.? Vital signs were taken at 1822 (Blood pressure 102/70, Oxygen 97%, Heartrate 62, Respirations 18). Color returned to client?s face at 1822 and client stated, ?I?m feeling better, I don?t feel dizzy any more and the headache is gone.? Client stated that he ate a small meal at 1400 and had not eaten since. PHN gave client a snack and a water to consume, which he accepted and consumed. Vital signs were taken at 1830 (blood pressure 110/74, oxygen 99%, heart rate 78, respirations 20). Client?s mother stated that the client had a cold last week. Vital signs taken at 1840 (Blood pressure 112/72, oxygen 98%, heartrate 76, respirations 20). Vital signs taken at 1849 (blood pressure 110/68, heartrate 76, oxygen 99%). Client stated that he ?did not have any reactions after his first dose of the vaccine.? Client also does not have any medical history, or allergies, and is not taking any medications. PHN advised client and his mother to follow up with his primary care provider and when to seek emergency care services. At 1849 client stated, ?I am feeling fine,? and left the facility unassisted with a steady gait.

This description was reported to this writer. A few minutes after receiving the vaccine and sitting in the observation area, the patient's father told the nurse that his son was feeling lightheaded, was sweating and appeared pale. EMTs were notified and approached the patient. The patient was laid onto the ground and his legs were elevated. EMTs did an assessment and vital signs. Reported that the patient was hypotensive. Dad stated that his son had not eaten anything yet today. He was then given juice and a small candy bar which he ate. Patient was soon transferred to a wheelchair and taken to a nearby cot to lay down again. Dad stated this son does not have any allergies. Dad remained at son's side until released to go home by EMS. Patient ambulated unassisted toward the car.

ABOUT 5 MINUTES AFTER VACCINE ADMINISTRATION, PATIENT EXPERIENCE POSSIBLE SYNCOPE AND POSSIBLE SEIZURE SYMPTOMS, PATIENT APPEARED TO LOSE CONSCIOUSNESS, AND HIS ARMS AND LEGS EXTENDED AND STIFFENED.

Dizziness, diaphoresis, bradycardia (47), momentary loss of consciousness, fell forward to floor, hit head. - woke quickly to verbal commands. Close monitoring, vital signs checked. EMS were called and transported Attendee to local hospital.

(DOB 04/08/2006) received the 2nd COVID dose (Pfizer, lot # ER8735 use by 07/04/2021 at 1400) at 1347. Approached EMT and stated, " I feel a little lightheaded." EMT immediately assisted the client to the anti-gravity chair and placed him in a semi-Fowler's position. The client was immediately given juice and a granola bar. RN assessed the client who was A & O x4 and denied SOB, abdominal pain, nausea/vomiting, or chest pain. Vital signs taken at 1356, BP-120/80, RR-18, HR-54, oxygen saturation- 98%. The client stated, " I feel a lot better." The client stated the last time he ate was breakfast, around 0700. The client denied any known drug allergies or having any chronic medical conditions. The client was speaking in full sentences with his parent, Fabiano Sarmento, and was in no apparent distress. Vital signs taken at 1401, BP- 120/80, HR-56, RR-18, oxygen saturation-98%. The client stated, " I feel better." The client reported being very active and reported playing sports regularly. Denied shortness of breath, lightheadedness, dizziness, chest pain, or vision changes. RN educated the client and about V-safe, PCP precautions, and Emergency Room precautions. The client ambulated with a steady and even gait to the car at 1426.

Pt had a syncopal episode w/ dizziness. Pt denies any PMH or allergies. BP 112/62, HR 75, RR 17. Pt given water and juice. VS stable, denies feeling dizziness, SOB. Physician at bedside - cleared patient and educated on ways to remain hydrated. Pt verbalized understanding. Total observation time ~30mins.

Pt c/o lightheadedness after getting the vaccine. Pt brought to triage area and was monitored. Pt given water. BP 93/57, HR 59, RR 18; 95/61, 65, 18; 105/70, 66, 18. Pt stated that he feels better and his symptoms have improved over ~15mins. Pt stable and released from vaccination site with father.

Fainted, fell off chair. Regained consciousness after 30 seconds. Reported that vision became blurry and then fainted. EMS responded with EKG and BP normal, Heart Rate was 106. Left facility after 40 minutes and walked home. approx 4 blocks.

Client received the COVID vaccine (Pfizer dose 1, lot # ER8736, expiration date 7/5/2021) at 1725 from RN. RN escorted the client to a chair in observation area at 1730. Per RN, the client's gait became unsteady as he ambulated and the client became unresponsive as he sat in his chair. The client was observed to slump forward, and he was caught by RN and his mother. EMT and RN responded at that time. The client remained unresponsive for approximately 90 seconds. While he was unresponsive, the client's breathing was rapid and shallow, and his pulse was thready. At 1732, the client became responsive and was drowsy but oriented x4. The client was transferred to the zero gravity chair at this time by RN and RPh. The client's vital signs at that time were: blood pressure 76/40, heart rate 64, respiratory rate 24, and O2 91%. The client appeared pale and diaphoretic. The client reported dizziness and blurry vision which had both progressed to "blacked out vision" before he went unresponsive. The client reported still feeling dizzy and having blurry vision. The client denied shortness of breath, chest pain, or headache. At 1737, the client's vital signs were as follows: blood pressure 102/52, heart rate 61, O2 98%. The client's pupils were equal and reactive to light. At 1738, the client was offered water, juice, and a snack, which he started to eat and drink. Per the client's mother, the client hadn't eaten prior to his appointment but was given water and a snack while in line for his vaccine. The client's mother stated that the client has no medical conditions or allergies and that he takes no medications. The client's mother stated he does not have a history of syncope. At 1740, the client's vital signs were as follows: blood pressure 96/48, heart rate 82. The client complained of feeling hot and he was provided with a cool pack. The client's color had returned to normal at this time and his pulse was normal as well. At 1745, the client's vital signs were as follows: blood pressure 96/48, O2 99%, heart rate 65. At 1750, the client stated that his dizziness and blurry vision had resolved. He was no longer diaphoretic. At 1755, the client's pulse was 74, his blood pressure was 102/52, and his O2 was 99%. Education was provided to the client and his mother by RN regarding primary care follow up and EMS activation. At 1800, the client's vital signs were as follows: heart rate 68, O2 98%, and blood pressure 106/58. The client was able to stand up without dizziness and ambulated out to the car escorted by his mother and EMT, ambulating with a steady gait.

Patient became dizzy and clammy but did not lose consciousness. He began throwing up. No sign of anaphylaxis. Remained pale, dizzy, for about 5-10 minutes. EMT called to scene but was doing a lot better by the time they arrived. Treated patient with cold compress face/neck. Dad stated he has anxiety and that this same thing happened previously after having blood drawn. He was able to walk out on his own with his father approximately 30 minutes later.

Patient came in for 2nd dose and was already anxious about getting the vaccine as he had the same fainting episode after the first dose. Rph and intern giving vaccine where both in the immunization room along with the dad. Rph counseled patient on what to do if feeling light headed and to help prevent fainting . Intern gave vaccine and within a few seconds he said it was getting dark and so immediately as the patient was sitting down I grabbed him gently from the front and his dad came and held him from behind. We both supported him as he was sitting down and within 10 seconds he came back and was fine. We then moved him to the floor lifted his legs and supported his head and he remained there for about 20 minutes. He then sat up slowly and we monitored him sitting for another10 minutes. He we doing well. My intern then proceeded to help his dad walk them out to the car. The dad did NOT want us to call paramedics as this was a known issues with his son fainting after getting immunizations. Made a follow up phone call with patient after about an hour and half and dad said he was doing great.

PATIENT BECAME FAINT, AND HOT AFTER RECEIVING VACCINE. PATIENT THEN WENT UNCONCIOUS, HIT HEAD ON FLOOR AND IS BELIEVED TO SEIZED FOR APPROXIMATELY 20 SECONDS. PATIEN THEN BECAME CONSCIOS, STATED HE WAS STILL FAINT AND LIGHT HEADED AND WAS SEATED UNTIL EMT ARRIVED. EMT CHECKED PATIENTS VITALS, RECOMMEND PATIENT BE SEEN AT NEARBY HOSPITAL TO EXAMINE PATIENT. PATIENT THEN WALKED OUT OF STORE WITH EMT PERSONELL AND GOT IN AMBULANCE

Client experienced loss of consciousness for approximately 20 seconds. Reported feeling weakness, light-headed. Client had loss of color to face, increased pulse followed by reduced pulse, that normalized on its own. After approximately 25-30 minutes client was feeling better, pulse normalized, color returned to face and he reported improvement in his symptoms.

Patient woke up dizzy in the middle of the night. He has never been dizzy before. He said his head hurt and asked for a tylenol. He took one and walked back to his room. Before he made it to his room, he blacked out and fell to the ground. Thankfully he did not hit his head. He fell on carpet but we did not know he passed out. He was out for about 1 1/2 hours before he woke up, found himself on the floor, and came back to the room to let us know.

On 6/8, he developed fever and chills. On 6/9, he developed shortness of breath, dizziness, increased HR while playing golf. Rested and symptoms improved. Resting HR at the time of my call with him was in the 80s, but he reported low 100s when he was playing golf. Symptoms resolved with rest and hydration only. He did not need to go to the ER or be hospitalized.

Patient received vaccine at approximately 1:10pm. Patient's mother called for help after the patient lost consciousness and fell, hitting his head on a shelf on the way to the floor at approximately 1:15pm. By the time we arrived, the patient was seated but slumped against his mother. He was extremely pale but conscious. We applied an ice pack to the back of his neck. The patient reported wanting to lay down and close his eyes. We instructed the patient to stay awake and engaged in conversation to ensure he stayed conscious. The paramedics arrived at this point and transported him to the emergency room.

The night of vaccine, started to have nausea, lightheadedness, muscle aches. Progressed to neck pain and headache, and later to shortness of breath , chest tightness, and diaphoresis.

Two minutes following vaccination, patient reports feeling light headed. Eyes began to roll back in head and arms became rigid and moved in a twisting motion. Patient non-verbal and slight coughing X4. Patient lowered to the supine position while supporting his head Pt had his eyes closed. Color became pale. This occurred for approximately 1 minute. Following this episode , patient opens eyes and begins to talk normally. Alert and oriented. Patient reports feeling very cold and begins to shiver. Nurse provides blankets to comfort. Nurse keeps client in observation for 30 minutes following this syncopal episode. Client drinks two small glasses of water. Denies being nauseous or hungry. Reports has not eaten today, but declines food. Patient has father and siblings with him to drive and care for him. Upon departure, patient reports feeling back to normal. Patient and family instructed to seek medical care if there are any changes.

6/12/2021: Nausea, slight fever, dizziness, black outs, fatigue, muscle aches By night time, rashes started appearing all over his body including arms and legs and all over his face. The rashes looks like big blotches/ map looking red marks all over his whole body. His face was all covered with the blotches of rashes including his scalp. He had no fever though. He was breathing normally but the rashes were very itchy. gave him double dose of Benadryl. Then switched to adult Benadryl but still taking double the dose. This went on for 2 days and no sign of improvement despite taking Benadryl every 6 hours. Patient was sleeping most of the days due to the medication. 6/15/2021: reached to Family doctor and explained the situation. doctor recommended to go to ER after seeing pictures of the rashes. Patient adamantly refuses to go. Doctor suggested take Allegra on top of Benadryl. Patient took Allegra 24D. Patient mostly sleeping. 6/16/2021: saw some slight improvements. Back and chest cleared of the rashes but still heavy on the arms, most especially the thigh area and legs. still taking Benadryl but only the normal dosage. 6/17/2021: All rashes are gone and cleared/

the son felt like he was gonna pass out and was pale; the son felt like he was gonna pass out and was pale; This is a spontaneous report from a contactable consumer. A 15-year-old male patient (reporter's son) received the second dose of BNT162B2 (PFIZER-BIONTECH COVID-19 VACCINE) via an unspecified route of administration at age of 15-years, on 03Jun2021 18:25 (Lot number was not reported) as single dose for covid-19 immunisation. The patient medical history and concomitant medications were not reported. Historical Vaccine included the first dose of BNT162B2 (PFIZER-BIONTECH COVID-19 VACCINE) on an unknown date as single dose for covid-19 immunisation. The patient experienced felt like he was gonna pass out and was pale on 04Jun2021 01:00. Events outcome was unknown. Information on the lot/batch number has been requested.

Patient had a near syncopal event approximately 5 minutes after getting the vaccine. He became shaky, weak, diaphoretic and felt like passing out. He was placed on the exam table and approximately 5 minutes later, he was feeling much better and was able to walk out of the clinic feeling like normal.

Patient is a 15 y.o. male presenting for 5 days of symptoms following his second dose of Pfizer SARS CoV 2 vaccine culminating in an urticarial rash which prompted him to present to emergency department. Patient received the second dose of the Pfizer vaccine on June 8 (first dose was administered on may 17 without symptoms). The next day (June 9) he complained of nausea and subjective fever. On June 10 he complained of chest "weakness" and shortness of breath intermittently. He had also mild light headedness but no syncope or palpitations. All those symptoms resolved by June 11. However, he had a diffuse rash on extremities, pruritic, and non-exudative.

2.5 weeks after second dose. Hives all over body. High tropinin T High sensitivity test. Confusion, dizziness, lightheaded, chills, hot flashes.

Seconds after receiving the second dose of the Pfizer vaccine, patient complained of headache and stomachache and lightheadedness. Patient's eyes rolled and he became limp. Patient's mother and the pharmacist helped him to the floor, and he regained consciousness seconds later. He was given water and a granola bar and said he felt better after a few minutes. This adverse effect did not occur with the first dose.

Approximately at 10:36 a.m., pt reported light-headedness after COVID-19 vaccine. Pt was sitting in observation area accompanied by mother. Pt slumped off chair, mother caught pt and laid him on floor. Pt quickly recovered and was A&O x 4. Pt did not hit head, though he reported a slight headache that resolved before he left premises at 11:05 a.m. Pt left walking with normal gait. Per mother, pt has history with prior vaccines & pt did not eat or drink prior to vaccinations that morning. Mother was advised to call MD provider with any further concerns.

Immediately following the vaccination, the patient told his father he felt dizzy, so he was directed to a chair. He told his father he could not see anything and was experiencing vertigo. His father then said his eyes and lips on the left side of his face seemed to be shaking as if he was having a seizure and he lost consciousness for a brief moment. When he opened his eyes, he told his father he was dizzy and his head hurt, but he could see again. I brought the patient some water to drink and took his first blood pressure reading at 10:42 am, which read 67/31, pulse 43. The patient mentioned that he hadn't eaten anything that day and that he was extremely nervous about getting the vaccination. His father got some orange juice and started to give him sips of it and he remained seated. The next blood pressure reading at 10:54 am was 115/59, pulse 76. The patient said that he was feeling nauseous, but less dizzy. I gave him two cookies to eat and took his blood pressure again at 11:01 am and it read 117/73, pulse 88. At this point, the patient said he was feeling much better and the cookies even made the nausea go away. The final blood pressure reading was at 11:13 am: 122/63, pulse 76. At this point, the said he felt completely better and father was confident in taking him home, so they left. I followed up with the father two days later on 6/25/21 and he said that he didn?t have any more problems after they left the store. That evening, patient's temperature went up to 99 and his head hurt with movement, but otherwise, he was fine and did not have any more symptoms after that.

At 1 am, my son presented with headache, dizziness, lightheadedness, shaking, ringing in ears, shortness of breath, and pallor. Vasovagal symptoms and near syncopal episode. Was able to get him to the floor to avoid a fall. Vomited several times. Severe vasovagal symptoms began to resolve within one hour. Didn?t check heart rate during initial near syncopal episode but was tachycardic after initial stabilization. Headache and fatigue persisted and is still lingering today.

Sweating, headache, nausea, light headedness, blacked out, dizzy, body aches - after second dose. Tried Ibuprofen so far. Waiting to see if there is relief. Can barely stand on his own.

Called ambulance and they came to check him out. His blood pressure was very low do they took him to the ER.

Patient received #1 dose of Pfizer vaccine today 6/29/2021 at 1:47 pm. Patient spent 15 minutes in observation room without exhibiting any signs or symptoms of any adverse reaction. Patient left observation room with parent and siblings and entered elevator near the check in desk of the clinic. Patient found laying in elevator on floor after staff arrived due to mother calling for help. Parent reports that patient passed out and fell to ground. Patient was conscious. Patient taken to observation room via wheelchair once determined that patient was fully awake. 911 called. Vital signs obtained at 2:10 pm BP 98/42 P 77 O2 Sat 100% . Patient transported to hospital with parent via 911 for further observation.

Was given the vaccine in the right arm and about 5-10 mins after being vaccinated as the patient was walking out of the consultation room the patient became lightheaded Patient sat down for about 15 mins and given 2 small cups of water to sip

Approximately 10 min after receiving vaccine, pt experienced vasovagal episode. Became faint, lightheaded, and complained of headache.

Pt. was observed leaning to right side with eyes closed for 5 seconds. Then open eyes and had no recollection of what had happened. Pt. states he is feeling fine denies and SOB of chest pain.

patient started to feel lightheaded. His mother asked how he was, his eyes rolled back in his head and he was unresponsive for a few moments. we moved him to the floor and called 911. his remained very lightheaded for the next 25 minutes until which time his mother decided to transport him to the hospital for observation.

7/4/2021: 15 year old male with autism and ADHD that presents to PICU from another hospital status post cardiac arrest times two. Mom states that patient has been having coughing for the past 2 weeks. Went to PMD on 6/25 who noticed wheezing on his exam. Ordered a CXR for 6/29. The next day, told mother of the results which showed pneumonia, "fluid in lungs." Started on albuterol q4, cefdinir, and OTC cough medicine with no improvement of symptoms. Began having decreased appetite, urine output. Had a couple episodes of post tussive emesis with blood tinged secretions. Abdominal pain started the day before admission. Patient continued having increased coughing and shortness of breath. On day of admission, patient was walking up the stairs to take a shower. He then fell and hit his head on the wall. Denies LOC at that time. Stated that he could not breath. Patient brought to ER. Patient went into cardiac arrest at 1840 until 1910. CPR was done, given epinephrine, and intubated. Then went into cardiac rest again. Placed on Vent, TV 450, PEEP 6, Rate 15, Peak pressure high 30s. Given ceftriaxone, vancomycin, toradol, 40mg lasix, 4mg zofran and 2L NS bolus. CBC, CMP, blood gas, troponin, pro BNP, RVP/COVID ordered. Started on versed (6.5), epinephrine (20) and norepinephrine (10) drips. Was then flown by helicopter to our PICU. Of note, patient had second dose of Pfizer vaccine on 6/19. Mom believes symptoms started just before his vaccine dose.

Patient received the Covid-19 vaccine and within 1 minute started to slump over with weakness and immobility. He had dizziness and was lightheaded. He complained of blurred vision. His mother was by his side. Pharmacist called 911 and paramedics arrived in about 5 minutes. Patient started feeling better but was pale and his vision was bad. Mom said he was dehydrated and at the lake all weekend. Paramedics took him out on stretcher to emergency room.

the patient was a 15 year old male , came in for the first dose of pfizer vaccine, after getting the vaccine , the patient suddenly fainted for a moment, but came back in less than a minute, he was still little dizzy for couple minutes after that, but was perfectly ok after 5 minutes, he also threw up, no other allergic reactions noticed, . mom had mentioned that he hadn't had anything to eat in the morning and he was really nervous too, that might be the reason she said. but we did monitor the patient for over 30 minutes and he was perfectly fine before he left

Patient experienced light headedness and fainting after receiving vaccine. Patient was give soda to drink and issue resolved. He had not eaten all day before vaccine and had swim practice for several hours

Patient received first dose of Pfizer vaccine and felt dizzy when walking towards the observation area. Pt had a vasovagal response and was helped to the floor by his father. Feet were immediately elevated and patient regained consciousness within seconds. Pt reported not eating anything that day, juice and crackers were given to pt. PHN Program Manager encouraged nursing staff to offer snacks to minors reporting that they have not eaten prior to vaccination. PHN Program Manager encouraged pt to notify nurses of 1st dose fainting experience prior to 2nd dose vaccination (at next appointment) and request cot for 2nd dose vaccination and recovery. Pt slowly sat up to snack and reported feeling " fine " after a few minutes. Patient ambulated out of clinic with father after 15 minutes of observation.

Systemic: Dizziness / Lightheadness-Medium, Systemic: Nausea-Medium, Additional Details: Patient's blood pressure was 114/80 pulse 68. Symptoms resolved after 15 minutes.

He felt dizzy 3-4 minutes after he received the shot. He vomited 7 minutes after receiving the injection. At 4 minutes after vomiting he felt better. He drank water and had cool wet clothes on his forehead and neck. He remained seated at our pharmacy department until he felt completely better. He left accompanied by his mother.

patient received the vaccine. patient was just fine for like 10-15 mins or so. When it was about time for them to leave, he stood up and thats when his mother called for help that he wasnt feeling well. he laid down on the floor for a little bit but then sat back up and sat back on the chair and said he was feeling better. i asked if they wanted us to call 911 and the mom said no. then, the patient said he had to vomit so we asked where he would like to vomit the bathroom is right next to him. patient chose to walk to bathroom and threw up a couple times. patient felt more dizzy/faint in the bathroom so the mother said please call 911 so we did. we sat with patient sitting on a chair for a couple mins and offered little cup of water. tried to talk to the patient and see if he was conscious. patient was responding to questions mom asked in another language. then, the paramedics arrived and put patient on a stretcher and said he seemed ok but was going to take him to hospital just in case. patient was breathing fine and not shaking.

The patient was given the first dose of the Pfizer covid vaccine, subsequently he was standing waiting with his mother for his younger sibling to receive his vaccine and after I had administered that, I turned around and saw that he was starting to lean over and his mother and I went to grab him as he went down and he fainted momentarily. We slowed his descent, but he had a small abrasion on his head (I think from the carpet). He said he felt dizzy and nauseous. I'm not sure if he was also watching while I was giving his sibling the shot and that exacerbated the situation. We had him checked out by EMT and he was fine after the initial fainting.

Vasovagal syncope at 7:30pm on 7/14. Patient was lightheaded immediately prior to event and woke up on the floor. HR 45, BP 94/46 immediately after event. EMS was called. Blood sugar was normal. Patient's family brought patient to ED for evaluation. Patient was back to baseline in the ED. Of note, patient also reporting right axillary swelling and pain after vaccination (that started on 7/13)

Patient had a vagal response caused by an anxiety attack due to fear of needles. Technician, gave the vaccine and then immediately called me in. The mother and son were worried about the needle disappearing due to the retraction mechanism. The son was loosing color in his face and becoming dizzy. I had him sit upright, talked with him for a few minutes, gave him Benadryl as a precaution, and checked his blood pressure. He was having no difficulty breathing and blood pressure was WNL. His mother said that he has passed out with needles before and that he had not eaten today. Eventually the patient claimed to lose his sight, and within 30s passed out. He was still sitting in the chair, bent over, vomited, and then immediately came back to consciousness. The patient then said that he felt fine and wanted to leave. I talked with him and his mother for around 20 minutes, had him eat a banana, and drink a full bottle of water. After the time, he felt fine and him and his mother left.

Client reported one hour after receiving first dose of COVID vaccine Pfizer he developed a headache and dizziness that lasted one hour. The client only ate breakfast the day he received the first dose COVID vaccine Pfizer. The client ate breakfast today. He was provided a snack of chips and started eating them. Vitals obtained on 07/14/21 were as follows: BP 118/80, HR 87, O2 98%. The client denied SOB after first COVID vaccine Pfizer. The client did not follow up with a primary care doctor or allergist. The client denies any chronic conditions or medications. No known allergies.

Patient seemed fine at the time of injection, upon leaving his seat and walking into the store he became light headed, pupils dilated, and fainted on the floor. This was approximately 3 to 4 minutes after injection. He seemed almost immediately fine, vision fine, and was able to talk and drink water. Monitored for 30 minutes after and no other signs / symptoms during this time.

Patient found kneeling on the floor with family member holding him. Other staff member in clinic was taking photos of the family and seen patient kneels quickly and was already assisting. Patient lost awareness of the surroundings for a second. Patient states " I got slightly dizzy after the vaccine and I dont know what happened after I stood up to take a picture." Pt found alert and verbally responsive. Patient was put into wheelchair and assisted to gurney. Placed in supine position with both lower extremities elevated. Patient denies dizziness, w/o headache, no nausea/vomiting. Vital signs monitored w/i 15 mins. 9:55 am bp-103/66,P-96,RR-18, o2 sat 99% 10:15 am bp-95/66,P-88,RR-17, o2 sat 99% , 10:35 AM BP-104/62, P-93, RR-17, 02SAT 99%, Patient stable, after care vaccination explained to family, patient sent home.

Patient passed out within a few seconds of receiving his 2nd Pfizer Covid vaccine and began what looked like seizing. This lasted for 3 seconds. Vaccine given 7/20/2021 at 6:30pm. No physical injury occurred. Patient gained consciousness within seconds. Patient still felt light headed, skin clammy to touch. Patient complained of pain in neck and kept stating he wanted to lie down. 911 was called within minutes and arrived within a few minutes. Paramedics took patient BP. Patient understood the questions asked was able to answer them all. Patient left on the ambulance with the mother.

Patient reports arm pain and difficulty moving arm since the vaccination, patient reported during second vaccine appointment and was advised that his arm would need evaluated, due to NP feeling uncomfortable administering vaccine with patient symptoms, at which time patient left clinic with father.

7/9/21 developed moderately severe throbbing headache localized to his forehead with moderately sever painful "tight" sensation in the muscles of the back of his neck and lower back. Pain persisted for ~ 3 hours ; Temp 101 Tylenol l taken but fever and pain returned after 3-4 hours. 7/10 awakened with nausea, dizzy/lightheaded , headache, vomiting. symptoms continued until 7/12/21. Viral infection suspected but nonetheless prescribed cephalexin "in case". Fever and headache continued with once daily eesis so came to ED for evaluation 7/14/21

At approximately 1445 EMT was notified by the mother of client, a 15yo male, that her son was feeling light headed. The client's mother stated that they were leaving the observation site after the client's 15 minute wait following a first dose Pfizer COVID vaccine (LOT EW0191, exp: 08/31/2021) when the client reported feeling light headed. The mother brought the client back to the observation area for evaluation. EMT placed the client in the zero gravity chair and at 1450 took a first set of vitals with client seated: BP 122/48, HR 75, RR 20, skins normal color for ethnicity, dry and warm. Lead RN assessed client who explained that approximately 10 minutes after receiving the first dose Pfizer COVID vaccine (given at 1425) he felt light headed, like a "buzzed" feeling. He did not mention the feeling to his mother or the EMTs. When he stood up to leave he stated the feeling got stronger. Client stated his last oral intake was a sandwich he ate at 1230. Client denied any other symptoms such as chest pain, shortness of breath, nausea, tingling of face, mouth or throat swelling. Client's mother denied any pertinent medical history, stated client had ADHD and anxiety, denied any daily medication use and denied any allergies to foods/medications/environment. Client stated he gets the same feeling when he gets the flu vaccine. Client was given a juice box which he drank and a bottle of water which he sipped. At 1452 a second set of vitals were taken: BP 136/64, HR 70, RR 20, skins warm, dry and normal color for ethnicity. Client advised to stay in observation and to alert if symptoms worsen or persist. At 1300 a third set of vitals were taken: BP 118/52, HR 80, RR 24. Client's mother confirmed with EMT that these are normal vital ranges for client. At 1310 client stated he felt better and symptoms of dizziness and feeling lightheaded had resolved. Lead RN educated client's mother on ED protocols. Client left the observation site with his mother on his own accord and with a steady gait.

A 15 year old male was given a Pfizer shot and 5 minutes later had a seizure for about a minute and then passed out for a few seconds and then woke up. He was very pale and dizzy. A nearby customer and pharmacy tech called 911. The patient did mention that he hadn't eaten or drink anything before he got the shot. The patient refused to go to the hospital.

Patient lost consciousness two times, one on 06/28 and the second one on 07/15. First time patient felt dizzy/nauseous, blurred vision (loss of color), loss of hearing, and then loss of consciousness. The patient woke up 5 minutes after. Patient felt slight confusion with no adverse effects after. Second time patient almost lost consciousness three times before finally losing consciousness. First time patient loses consciousness for less than 3 seconds before waking up. 3 minutes later patient felt dizzy before losing consciousness for less than 3 seconds. 6 minutes later patient felt dizzy before losing consciousness for less than 3 seconds. Less than 1 minute later patient felt dizzy and nauseous before losing consciousness for 1 minute. After waking up patient felt slight confusion with no adverse effects.

Site: Itching at Injection Site-Medium, Systemic: Allergic: Difficulty Swallowing, Throat Tightness-Medium, Systemic: Allergic: Itch (specify: facial area, extremeties)-Medium, Systemic: Allergic: Rash (specify: facial area, extremeties)-Mild, Systemic: Dizziness / Lightheadness-Medium, Systemic: Shakiness-Medium, Systemic: Tingling (specify: facial area, extemities)-Medium, Additional Details: Pt exhibited rash on both arms, that he noticed after 15 min wait, observed pt, rash itch worsened, 50 mg benadryl chewed, within minutes pt complained of facial and mouth numbness/tingling, then a couple minutes later difficulty swallowing. 911 called, 1 dose epipen administered in right thigh. mother did not wait on 911, took son to ER. mother Chambree called to say pt was fine.

Experienced a vasovagal syndrome 5 minutes after COVID vaccine. c/o feeling dizzy, denied itching, shortness of breath, nausea or headache. On exam by Health Care Provider, vitals BP 90/60 rt arm, 106/62 left arm, pulse 59-71, resp rate 20, glucose 102, temp 97.3 with 98% pulse ox. PT transported by EMS to medical clinic for evaluation, In medical clinic, given Pediatlye, monitored and released after feeling better. COVID vacine given on Jul 12th. On Jul 15th, pt c/o flu like symptoms, rapid test neg-fluids given. On Jul 16th, pt temp was 102, HR 98, BP 114/52 and tested positive for COVI. Placed in isolation and will be released on jul 27th

Patient received his 1st COVID (Pfizer) vaccine and he bled more than usual. Patient reported feeling faint and nauseous. While monitoring patient, he threw up a few minutes later. He felt better shortly after. We kept in the pharmacy area for observation for 30 minutes to ensure that he was ok before he left. Patient was feeling fine when he finally left.

Patient complaint of dizziness, shuddered, and passed out per sister. PT passed out in car. EMS arrived and assessed PT. Parent was present. VS 108/40 , HR 49 , 02 Sat 99% on RA. PT provided cold pack and water. PT's Mom declined EMS transportation to ER. VS WNL per EMS when PT left site.

Systemic: Dizziness / Lightheadness-Medium, Systemic: Exhaustion / Lethargy-Mild, Systemic: Fainting / Unresponsive-Mild, Additional Details: pt's dad noted he was well when recieving vaccine however when they were in line in the coffee shop area of the store pt leaned against dad with light headedness as if to faint. Store staff called 911 and I rushed over and took pts vitals which were normal with bp 115/72. Pt sipped some water and when I asked if he had eaten today he responded no (215 pm) his dad also reported he had just woken up and doesnt follow a sleep schedule. I was able to offer him some juice which improved his weakness

SUBJECTIVE: Pt. is a 15 year old male here vaccinated at our community vaccine event at high school last evening and received the Pfizer vaccine Approx 10 min after receiving the vaccine the patient reported: weakness and lighheadedness At approx 10 min post vaccination he was still sitting in the chair he was vaccinated in. When I came to him he was clamy and pale and felt weak and lightheaded. He also c/o of feeling tremulous This Hx of was obtained by pt and pt's mother who was with pt at the vaccine event. Pt was feeling anxious but well prior to the vaccine. He last ate approx 5 hrs prior to vaccine and had hamburger and root beer. He had no significant activity during the day prior to the event. He did wake up approx 4 hrs earlier then his usual time on day of event because of school orientation he had that day. Pt and mom denied any prior hx of similar reactions after vaccines Pt reported it "feels like just got a shot" No nausea, itching or resp distress No PMHx -Per pt. has been on IEP for adhd No current medications NKDA SocHx: pt denied any recent drug or etoh use Sudden onset of symptoms/signs: yes Rapid progression of symptoms/signs: Rapid progression that quickly improved OBJECTIVE: General appearance: pale and clammy Skin:no rashes Resp: CTA bilat CV: RRR Vitals Vaccine given at 5:58 pm 6:05 pm BP 87/50, P 83, pox 98%- pt was given water 6:08 pm BP 134/84, - coloring in face improved and pt began to feel better- pt was alert and oriented x 3 at this point 6:13pm BP 130/66- pt was given granola bar and 2nd bottle of water BS: 118 DISPOSITION: After sitting in chair for approx 35 min, drinking two bottles of water and two granola bars pt was feeling better, appeared better with normal coloring and not clamy and had normal vitals signs. He and his mom felt comfortable going home to rest. I reviewed warning signs with mom to call or go to ED for I called the next day (7/29) and spoke with his mom and she stated pt was doing better after eating dinner and was feeling well this. She plans to call pt's pediatrician to coordinate his 2nd dose in their office or affiliated clinic.
