Patient was in observation area and started feeling dizzy with difficulty to see 10 minutes after vaccine administration and decided to go from sitting on chair to sit and then lay down on floor on top of her jacket. Observation room clinic staff noticed and notified RN lead. Patient was attended to and noted that she was alert and oriented c/o dizziness. Patient transferred from floor w/ assist to wheelchair and then to side observation area where her VS were obtained: 0850 BP 79/39. HR 62, O2sat 100%. Mom accompanied patient and stated patient did not eat this morning. Patient given water and crackers. VS 0853 BP 98/56, HR 71 and O2sat 100%. Patient stating that vision is better, continued eating and drinking. VSS 0855 BP 99/64. HR 68,O2sat 100%. Patient stating vision much better and dizzy feeling gone. Patient stood up and did not experience any symptoms, walked to observation room with mom and RN and continued to sit for an additional 10 mins. Patient left with mom at 0915 stating she is back to normal. Encouraged to have patient eat and drink prior to next vaccination appointment.

Approx 6-7 min after receiving vaccine in left arm. Patient's mother expressed concern about her daughters reaction. She complained of headache, stomachache, weakness in her legs and her eyes rolled back in her head and her head dropped.

About two minutes after she received her shot she told us she was seeing black spots. Then she told me she couldn?t see and she passed out. She started to regain consciousness and a little bit of blurred vision and then she began dry heaving. She was clammy and sweaty at that point but continued to improve after she drank some water.

Approximately 20-25 min after vaccination, after leaving 15 min observation time, daughter started complaining of a ball in her throat and "closing sensation"

Patient received in RN triage area, post-COVID vaccination administration. Patient being observed for 15 minutes as recommended per CDC guidelines and facility policy on post-vaccination RN monitoring. Patient sitting in chair and reported feeling dizzy/lightheaded, blurred vision, ringing in the ears, patient noted pale, clammy, and diaphoretic, patient reporting feeling short of breath. Rapid response called. 14:05 Vitals: 75/34, HR: 54, 97% Patient placed on gurney, in the supine position. Patient and mother denies adverse reactions to vaccinations in past. Denies any medical history besides asthma. 14:10 Vitals: Bp: 105/53, Hr 83, 100% on 2L. Patient now denies any dizziness/lightheadeness, blurred vision, ringing in ears, and shortness of breath. 14;15 BP109/65, HR: 72, O2: 100%. COVID vaccination fact sheet and discharge information provided to patient on D/c from clinic, patient advised to review. Verbalized understanding. Appointment for 2nd COVID vaccination provided to patient. All questions and concerns addressed. Advised to follow up as needed. Patient departed with mother at 1435 symptom free.

child became unresponsive, clenched teeth, unable to stand alone, vomited, eyes dilated, unable to speak, headache, called 911 was transported to the hospital emergency room

Patient began to experience symptoms consistent with an upper respiratory infection one day after getting 1st dose of COVID-19 vaccine. By day four, she developed an ear infection and began antibiotics. It is now exactly one week after the shot and 3 days after starting antibiotics and she still feels miserable. She?s losing her voice, throat is sore, and both ears feel infected.

Swelling of the upper left lip on 05/20/2021 noticed when she woke up at 7am. No signs or history of trauma. Gave Benadryl 25mg liquid orally and iced the area. A second dose of Benadryl was given at 1pm as the swelling had not subsided. By evening her lip was normal. When waking up on 05/21/2021 she noticed a approx 1cm ulcerative type lesion on her inner upper lip. The lip was swollen again, but slightly less so. Repeated Benadryl 25mg three times that day at 6 hour intervals. Started on triamcinolone 0.1% cream prescribed by her pediatrician that evening. On 05/22/2021 the lip was not swollen and the ulcer appears to be healing on the upper lip, but a small portion of her lower left lip now shows an approx 1-2mm ulcerative lesion. Triamcinolone 0.1%cream applied 3x daily. She noticed a irregular confluent red rash approx 4cmx2cm on her right mid abdomen on Tuesday after swimming, but unsure if this is related. She had her regular maintenance allergy shots (one shot in each arm) on Tuesday at 12:15pm.

My 12 year old daughter has reported having twitching of the head and right eye. She reports it started last week and continues to this day. The twitching of the head happens as often as every hour or more. The twitching of the right eye is rare. Depending on how fast the twitch happens with her head pain is associated with it.

AE: light headed, slight ringing in the ear(resolved), dizzy (at 10:40 am) Tx: n/a, just food and drink Outcome: juice, crackers helped resolve AE. pt was discharged home.

Ringing in ears, blurry vision, difficulty breathing, tingling in left foot. Denies headache, chest pain/tightness. VSS. RR 16. O2sat 98%. HR 79. Symptoms improved with rest. Discharged with parent in stable condition.

Patient stated was having blurred vision and couldn't see. Skin appeared pale, denied trouble breathing, no sign of swelling, rash, shortness of breath, cough. Patient layed down on the floor, with feet elevated. EMTs arrived, evaluated patient, patient began to feel better and was walking around okay.

Systemic: Fainting / Unresponsive-Severe, Additional Details: PATIENT WAS GIVEN A PFIZER COVID 19 VACCINE INSTRAMUSCULARLY. IMMEDIATELY AFTER BAND AID WAS PLACED, PATIENT APPEARED TO BE FROZEN FOR SEVERAL SECONDS AND WOULD NOT RESPOND TO MY QUESTION. MOM ASKED QUESTION AND HELD PATIENT, AT WHICH TIME THE PATIENT COLLAPSED AND HER EYES ROLLED BACK. SO SHE PUT IN A SUPINE POSTION AS I RAN AND CALLED 911. PARAMEDICS ARRIVED TO THE SCENE AND MONITORED HER VITALS FOR NEARLY 30 MINUTES. THEY OFFERED TO TAKE HER TO THE HOSPITAL BUT PATIENT'S MOM DECLINED.

patient has experienced three episodes of bloody nose and one of prolonged bleeding gum (after tooth loss) in the week since receiving the vaccine (not previously experienced these issues before)

light headed, headache, dizzy, vomiting ,sore throat, ,hard to swallow, weak, felt like someone sitting on her chest lost sense of smell and taste. Taken by medics to local hospital, , Placed on IV , GI cocktail, chest x rays 2 views, EKG,

She started to get a sore throat and drainage the day after the first dose of the vaccine. That day she had headache, sore arm with earache and joint pain. Those symptoms continued for several days with some loose stools which lasted for two to three days with abdominal pain which persisted for a little over two weeks. We visited the doctor because of ear and throat pain and nausea. All tests were negative, but the symptoms improved slowly but they are totally resolved right now.

Delayed reaction to vaccine on 5/29. Woke up from a nap and lips were swelled. Lips were hurting but no other symptoms present. No trouble breathing or itchiness of throat or mouth present. Advised pt possible angioedema and recommended to take benadryl 25 mg every 6 hours for 24 hours.

Patient became extremely lethargic. Vision was very blurry. Blood pressure registered 89/64 with pulse of 72bpm. Patient unable to move extremities

Neck pain and spasming - sharp shooting and progressing to ear and temporal pain. Initially occurring a couple times a day and now more frequent. Also, originally started on just the left side of the neck and is now occurring on both sides.

The morning after the 1st COVID vaccine, patient experienced a sudden onset of feeling clammy, pale and narrowing vision. Near syncopal episode. Resolved after lying down and resting for about 10 minutes.

After the second dose she had immediate pain at the injection site and over the next 24 hours she developed: A fever of 101.4 severe abdominal pain and chest pain that made her feel like her heart was being pulled out painful electric shocks down her neck and spine that made her walk hunched over numbness and swelling in the arm she got the shot pain in her fingers and toes that turned white and were ice cold to the touch Over the next 2.5 months her abdominal, muscle and nerve pain became unbearable plus she developed new symptoms: Fatigue gastroparesis, nausea and vomiting Eventually she couldn?t even swallow food or liquids without immediately spitting it up. An itchy rash on her arms peeling skin on her feet Her menstrual cycle lasted a month with large clumps of blood She had unexplained painful cysts vision problems headaches erratic blood pressure and heart rate memory loss, mixing up words and brain fog Dizziness, fainting and then nonepileptic seizures that we suspect were from lyrica verbal and motor tics loss of feeling from the waist down, muscle weakness, abnormal gait and eventually she wasn?t able to walk at all urinary retention From the day she got her 2nd dose to today we took her to the ER nine (9) times and she was admitted to the hospital a total of 3 times totalling 2 months. The last time she was admitted to the hospital she could not walk, was unable to feel or move below her waist, threw up anything she tried to eat or drink, had tachycardia and her blood sugar was at 47. Once she got an NG tube and was stable they transferred her to Inpatient Rehabilitation and she was just discharged on June 1st. Today she is able to walk with a walker and take care of herself but she still has little to no feeling below her waist. She still has an NG tube for nutrition and continues to have GI and urinary retention problems.

Patient was very nervous about the receiving the 2nd dose. We had a in depth conversation about side effects because she had a headache with the 1st dose. Mom held her hand. A few minutes post, her sister saw that she was slumped over & her eyes had rolled to the back of her head. We tried to wake her up & she was very lethargic. I gave her 2 small bottles of water, pb crackers, 3 hard candies & elevated her feet. Mom said we didn't need to get medical attention. After about 10 minutes she was still very lethargic & mom said yes to EMS when asked again. Another mom came over & helped us lay her on the floor to elevate her feet above her head. Her husband came in & assessed patient. Her pulse was 60 & when he asked her to squeeze his fingers & press against his feet it was week. She told us that she hadn't eaten a full meal since the night before, only had watermelon as a snack. The ambulance came & took her.

Pt has an allergy to peanuts. Pt c/o itching in the mouth 15mins after vaccination. BP 111/71, HR 84, RR 16, O2 sat 99% on RA. Pt did not disclose reaction to first dose. Pt given 25mg Benadryl PO. Pt stable and released from the vaccination site.

Pt has an allergy to peanuts. Pt reported itching in the mouth after leaving vaccination site (~1hr post vaccination). Pt took Benadryl at home and symptoms resolved. Pt disclosed this reaction after receiving second dose.

10:20 - client feels dizzy, sees spots, layed client down on the floor with elevated feet on the chair. NP called, EMS called. 10:21- EMS and NP arrived. 10:22- b/p- 106/69, hr- 98, o2- 100%, Awake, alert, and oriented, pallor present. Laying down with lower extremities elevated on chair, continued to monitor. NP assesses pt symptoms and capillary refill. Pt stated she felt fine at the time. !0:27- Vitals taken laying down. BP- 101/66, P- 89, O2- 100%, continued to monitor. 10:30- pt is moved to sitting position on the floor. BP- 98/57, HR- 104, O2- 100% encouraged fluid intake. Pt verbalized feeling better. Team continued to monitor. 10:35- Pt assisted to chair, BP-102/67, HR- 101, O2- 98%. NP continues to assess. Pt verbalizes feeling fine. 10:38- pt denies dizziness and seeing spots. HR-97. Continued to monitor. 10:39- assisted pt to standing position. BP- 94/54, HR 80, O2-97%, pt states she feels dizzy, pallor on upper extremities. Pt is immediately assisted back to the floor in a lying position with lower extremities elevated. Team continued to monitor, pt remains AOx4. 10:45- Assisted pt back to chair after expressing she wants to sit and feels fine. Encouraged fluid intake. Verbalized feeling better BP- 99/59, HR 94, O2- 98%. 10:50, temp- 96.6 F remains on chair sitting up drinking water and gatorade, team continues to monitor. 10:54- assisted pt to standiong position, pt verbalizes feeling "good," denies dizziness and seeing spots. BP- 99/54, HR- 107, O2- 100% 10:57- Client continues to drink water and gatorade. HR- 96, O2- 100% 1100- Hr- 82, O20 97% verbalizes feeling much better. 1104- continued to monitor and pt verbalizes feeling better without dizziness. 1105- Vitals checked in a standing position, BP100/65, HR-101, O2- 100% 1107- NP and EMS clear pt for discharge. Pt is clear of complications, and ambulated to car with staff and family with steady gait. 10:45

This was my daughter's 2nd dose of the vaccine. After she got up on the morning of the 12th, I heard her in the shower and went to check on her. Her face looked gray and she said that she needed to sit down. After sitting for a bit, she was able to stand up, but then proceeded to fall forward, as if she were passing out. I was able to steady her and sat her back down. Her eyes looked like they were staring off into space. She said that she couldn't see me and everything was "purple and yellow." This lasted a few minutes and then I was able to get her out of the shower and dressed. She then began to regain some normalcy and was fine after that. She ate breakfast and rested. She didn't have any more episodes after that, but I could tell that she wasn't as perky that day. This episode was definitely not normal for my daughter, so I felt that I should report it.

metal taste in mouth with all foods the 1st day, but now metal taste in mouth only with proteins, meat, chicken, eggs, and cannot eat them, very little with fish, none with shellfish,

immediately after receiving vaccine patient slumped forward in the chair and lost consciousness. Her mother and I both tried to lean here back in the chair she was sitting in. she jolted a few times and her eyes rolled backwards. I asked her mother if she has ever had seizures, she responded no. Her mother stabilized her and I ran to get the NP next door, by the time we returned she had regained consciousness, but was a little confused and very pale. The NP checked her out and found out she did not eat breakfast. We gave her some apple juice. After 15-20 minutes her color had come back, she felt fine, and she was able to leave. Her mother told me later that both patient and her mother have had syncope events in the past.

Pt was nervous about getting the COVID-19 IMZ. After the shot she was sitting next to her mom and told her she was feeling thirsty. Her mom then put her arm around the patient who then fainted. The mom held her up and gently laid her on the floor and raised her legs up on the chair. I got the patient a bottle of water and the color came back into her face. She laid on the floor for about 15 minutes and then waited another 15 minutes after that and was feeling fine after that and she went home with her mom.

Right sided intermittent eye pain, ongoing on a daily basis since June 10, 2021. Treated with warm compresses, ibuprofen and acetaminophen. Unresolved as of this report.

Client reported she self-medicated with Motrin and symptoms resolved the next day (2 days after vaccination). Client denied having any allergies or chronic conditions and she did not follow-up with a physician regarding side-effects. Vaccine site lead provided education to client and mother on COVID-19 vaccine side-effects and encouraged to report any additional side-effects on VSAFE. Clinical coordinator was consulted and provided approval to administer 2nd dose with a 30-minute observation period.

Within minutes after receiving 1st dose of Pfizer-BioN Tech covid Vaccine 0.3 ml IM, patient complained of dizziness and fell to the ground. Patient was very pale and cold to touch, not being responsive well. Patient was losing consciousness and having difficulty keeping her head up. Patient was moved to lie down on the floor with legs up. Cold water was offered and patient took a few sip. EMS was called. Patients eyes rolled back into her head and lost consciousness/possible anaphylactic shock and not being responsive. Epipen 0.3 MG IM administered. Patient regained consciousness. Nurse practitioner was called and vitals including BP and oxygen level was performed and found to be stable. EMS arrived in few minutes and vitals were checked again by EMS personnel and patient was transferred out to nearby ER. After 3 and 1/2 hours at the ER for observation, patient was discharged.

Approximately three minutes after administering the vaccine we heard the patient's mother speaking her name several times with no response. One of the technicians was in the area and observed her with her arms tucked in front of her and her feet were moving back and forth in a repetitive motion with her eyes rolled back. After a few seconds she ceased that and said "I was sleeping" We called 911 and EMS came to check her out

A 12-year-old female patient (non-pregnant) received second dose of bnt162b2 (PFIZER BIONTECH COVID-19 mRNA VACCINE, Solution for injection, Lot number: EW0187), via an unspecified route of administration, administered in Left arm on 06Jun2021 at 09:00 AM (at the age of 12-year-old), as single dose for covid-19 immunization. The patient's medical history was none. The patient's concomitant medications were not reported. The patient previously received first dose of BNT162B2 (PFIZER-BIONTECH COVID-19 mRNA VACCINE, Formulation: Solution for injection; Expiry Date: unknown, Lot number: EW0180), via an unspecified route of administration on 25May2021, 09:00 AM administered in left arm as single dose for COVID-19 immunization. The patient did not receive any other vaccines within 4 weeks prior to the COVID vaccine. Prior to vaccination the patient was not diagnosed with COVID-19 and since the vaccination the patient was not tested for COVID 19. On 15Jun2021 at 13:00, the patient experienced injection site sore arm four hours after vaccination/ arm extremely sore; she was very tire 10 hours after chills; had fever; experienced shallow quick breathing, heart racing, dry throat; and she was very uncomfortable. On the same day, she fell asleep about 11:30pm and threw up at 3:00 AM. On 16Jun2021 (on next day), she had headache and still on day 3. Therapeutic measures were taken with Tylenol, cool compress, hydration as a result of reported events. The outcome of event was resolving. Follow-up attempts are needed. Further information is expected.

My daughter experienced on day of her 2nd vaccine, feel nauseated in the evening time, then the second day. Felt very lethargic, fatigue, laying in bed all day, sleeping. She still had nausea also experienced chills, highest temp 99.8F. Her arm pit on the right side where she got the vaccine was really sore. She basically like within a couple of days she started her period cycle again and it was very heavy and lasted one week and half almost two weeks. On 06-08-2021, at about 10pm, she was brushing her teeth when she saw black spots and she went to tell me. At about 11:30pm that night, she squatted to pat our dog and when she got up and she felt really dizzy, lightheaded so I got concerned. Her symptoms persisted and I scheduled an appointment with her pediatrician on 06-15-2021. The pediatrician ruled out anemia. Her pediatrician said she has Postural orthostatic tachycardia syndrome. Her pediatrician said that this is something manageable, she needs to strengthen her calf muscles and drink lots of water. They monitored her heart pressure and blood pressure. Her heart rate had increased to 40bpm. I also gave her Ibuprofen medication after the vaccine. Leading to today she has been having dizziness and light headiness every single day. My daughter is still experiencing symptoms, dizzy and light headed today. For her 1st vaccine- within few days after her 1st dose, she experienced when using the treadmill, she would feel very dizzy. I did not think anything of it at first and this happened three times then it stopped. She also had a sore arm.

12 year old patient received 1st dose of Pfizer COVID Vaccine. After receiving the vaccine and waiting the 15 minutes, the patient and her father left the pharmacy area and began to shop in the store. While walking the patient stated that her stomach was hurting and she felt her eyes "look up and then down" and then she started to fall. Her father was able to catch her before falling and carried her back towards the pharmacy area. Patient was able to walk before completely reaching the pharmacy area. Once she was in the pharmacy area, the pharmacist was about to administer another vaccine heard a member state that she would call 911 for the patient. After then the pharmacy went over to the patient and her father and noticed the patient was sweating and immediately gathered a cool compress and provided care to the patient. Patient stated that she received her shot on an empty stomach and the father went to get the patient food. EMT were called and arrived and asked patient a series of questions in which the patient was able to respond to. After about 30-45 minutes, patient had eaten and stated that she felt better and her and father left the pharmacy.

A 12-years-old female patient received bnt162b2 (PFIZER-BIONTECH mRNA COVID-19 VACCINE, solution for injection, Lot Number: EW0178; Expiration Date: 31Aug2021), dose 2 via an unspecified route of administration, administered in Arm Right on 08Jun2021 16:00 (age at vaccination: 12 years) as DOSE 2, SINGLE for covid-19 immunisation. Medical history included, meningitis from an unknown date and unknown if ongoing at 7 years old. The patient's concomitant medications were not reported. The historical vaccine include COVID-19 Vaccine on 18May2021 for COVID-19 Immunization (1st dose Expiry Date of Covid 19 vaccine: 31Aug2021, lot EW0168 ,anatomical location: Right arm) and had she had itching on the hands. And other vaccine includes flu shots (she does the flu shot every year) for immunization. The patient experienced Meningitis pain on unspecified date, lip swelling and numbness, itching on face, hands and feet / caller states then her daughters hands and feet began to itch in Jun2021. She has pressure on her chest on 09Jun2021. She had pressure behind her eyes, muscle pain,she had not slept, fever, headache, the chills , sore throat, nauseous, caller states she was assuming hives, inflammation stayed around awhile , body aches / pain through her head and back down her neck, trying to push her eyeballs out and it is really painful , she was slumpy, swelling from the digging and scratching, her hands began to swell yesterday in Jun2021. she woke up and said her face felt funny on 09Jun202. she was just not feeling right and couldn't walk by herself on Jun2021, Caller relays her daughter will be (withheld) . HCP: Caller states she called the pediatrician office and 1 was by phone and she was seen by this provider yesterday and she encouraged her to reach out to us as well. Mom states she was aware of the possible symptoms to occur and her daughter came home Tuesday perfectly fine and went to bed and woke up with the typical symptoms, she had not slept, fever, headache, the chills, sore throat, nauseous and had all the symptoms pretty much. States she didn't have anything weird going on during the observation time the day of the vaccine. Caller relays her daughter didn't have anything weird after her 1st dose. Caller states she took a nap Wednesday about 24 hours after her 2nd dose and she woke up and her lips had begun to swell and she woke up and said her face felt funny and it was itchy. Caller states she was assuming hives. Caller states that there was nothing concerning as it wasn't in that initial 15minutes to 4 hour and that this occurs 24 hours after and that was a concern. Caller states then her daughters hands and feet began to itch and that was kind of along the same lines and said hives and she would itch itch itch and there was swelling from the digging and scratching. Caller states she has numbness in her lip with the swelling. Caller states the other symptoms that are bizarre is more yesterday and it started some on Wednesday she had a pressure in her chest and this was a child, her daughter was very strong, she got up yesterday and tried to do school and had a fever of 102 and Wednesday evening was so sick with symptoms and she still tried. Caller states the pressure in her chest is concerning and told them yesterday and she needed to be seen. States her daughter described it to the doctor that it felt like someone was sitting on her chest sort of pressure and on a scale of 1-10 she was a 7. Caller provides that not at any time did she experience trouble breathing and she never saw her lose or get weird coloring. Caller states she called her doctor Wednesday afternoon and talked to another doctor, that she called the help line after hours as they close 4:30 or 5pm. States she was told it was good news that she did not have trouble breathing but to have it at this point was weird and no one knows what to expect and everyone is different. States she was told to do Benadryl and continue with pain medication for fever and discomfort. Caller states her daughter was miserable the rest of the evening. Caller relays that the swelling subsided and the inflammation stayed around awhile and yesterday she woke up better with no nausea and the common symptoms are better and she still had the fever. Caller relays that at about 9:00 she began to have a problem where she said that behind her eyes she had significant pressure and that scared the caller what with the brain and swelling. Caller states her hands began to swell yesterday, like her lip and her fingers are itchy and swelling and it's constant itching and the pressure on her chest yesterday, that those 3 things freaked the caller out and she called the pediatrician and they said to have her checked out to make sure as nobody wanted to rush her to the ER as she was not struggling to breathe she was just not feeling right. Caller states the pediatrician did her research about heart inflammation and listened to her heart a couple of times and said her best observation for rubbing sounds was she did not hear any. Caller states when her daughter was younger she had meningitis and went through a whole lot with that and caller states she does not immediately rush to the hospital as that brings back emotions with that. Caller states the doctor said she didn't see any immediate distress and would do an EKG or they could wait and see, that she thought the pressure might be like body aches and her daughter had laid in bed all day the day before and had a time with it. Caller states she couldn't walk by herself and she was out of it. Caller states she could do the EKG or wait until today and they chose to wait. States that today she seems better and the only thing today is the continuing pressure behind her eyes that her daughter described as it felt like someone had 2 spoons and was trying to push her eyeballs out and it is really painful. Caller relays her daughter has been through meningitis pain and has pain through her head and back down her neck and the concern is swelling. Caller states it could be a weird reaction and the doctor thinks it is a weird headache she is having and is still doing ibuprofen. Caller relays that her chest is not nearly as heavy today and she has taken Benadryl again for itching and swelling. States the doctor says they don't know what did it because the risk was there and the benefit and don't want to get reactions. Caller states she had no problem with getting the shot, that weird things happen and the pediatrician is saying it can happen but doesn't know. States it is kind of scary. Caller states they checked her out and they checked her oxygen and blood pressure and that made the caller feel better. States that 9:30-10 last night after she got home she was slumpy and her head was hurting and she had chest pressure. Caller states with the hand swelling, that the caller herself went through reaction and hives and the doctor had asked if her daughter had anything new introduced and no nothing was and her daughter didn't seem concerned to share this but after her 1st dose her hands itched and she didn't pay it any mind. States she had no face swelling. Caller states she is wondering if those little hand itches after the 1st dose she didn't tell about till now was that a little bit of a sign that something else was there. States she didn't mention it after the 1st dose and was good to go. States after the 2nd dose she said mommy my hands itched after the 1st dose, her lip didn't swell and she told the doctor it's not conditions on the list of symptoms and she didn't have breathing issues. Caller states it's a non severe reaction and is assuming to dose and said yesterday if she gets a booster what does Pfizer advise. Caller states they have all intention of getting what she needs in the future. Caller states she wants to make sure she doesn't have worse and the lip swelling was freaky. Caller states she thought her daughter was out of the woods after the 20 minutes waiting and was home and then turned. Caller states it was pretty straight forward and knew what the expected symptoms were and they were fine and knew she was having the exact ones on the list and was not feeling well and had a fever. States she was thrown for a loop when others came up and the fact that they were 24 hours out, that she got the 2nd dose 4-4:15 on Tuesday and these evolved well after that time. Caller said they said to report this and her hope is that they can help if someone else has this issue. States when she said she was laying there my hands did itch after the 1st but not like this to this degree. States she never said anything and caller didn't know. States she knows as a parent there are certain questions to ask and warning signs of potential allergic reactions. Caller states she is assuming this is some sort of allergic reaction, not the worst kind but pressure in the chest and other concerns about her heart having some sort of issue she knows have been reported.Caller states that today she could have done the EKG for more information but based on her listening and what she wanted to thinking the body aches and extensive issues with that she is glad she waited. States she was laying in bed at 6:30 and asked her if she wanted to get the EKG and she said she was afraid of going to the hospital and it's not fun but would give answers if there is something else and she laid in bed until 9:30-10 and gave her more Benadryl and Ibuprofen and continued with that for the discomfort behind her eyes and the doctor is thinking it's body aches. States the fever subsided yesterday.Caller states she would like to share that her daughter is a tough kid and tough kids can be taken down too and may not pay attention. Caller states she doesn't over react but she is aware and if she had told her about her hands she would have thought allergic reaction, mild one and brought her to the doctor or at the 2nd vaccine and she didn't say anything until after the fact. Caller relays her daughter has never been through hives and the caller has and has a lot of seasonal and environmental allergies and the caller was that kid. Caller states with her daughter not having seen her lip swell and her eyes started hurting and she started thinking the worst. States the doctor pushed on her and checked her lymph nodes and did a thorough exam that lasted about 30 minutes and nobody knows. States she appreciates all that Pfizer and the other companies have done, that we need this.Caller states her daughter goes to the pediatrician in 2 weeks for her annual check up. States the doctor yesterday said her body responded and had a hard time with it. States she does the flu shot every year and this was weird and she wanted us to know and hopes this report can help. Caller states her daughter is a tough kid nobody knows and today she is better, she has a headache behind her eyes still there.Caller relays that Friday she had her annual eye check up and it's not a vision thing, that was fine until Wednesday and other other stuff and yesterday with pressure and she is sure it's residual and her body adjusting and if she can help with letting other people know.Caller relays if it comes back she is going to do the EKG, that she is not the parent to say it is nothing and will follow through with Pfizer also. Caller states she didn't have anything weird for heart beat and no rubbing sound and no signs and she thinks it was issues from body aches and hopefully she is on the up and up and nobody will have to leave and go to the doctor today. And didn't think because it was not an immediate issue with her breathing and nothing like that and didn't think it would be an issue with a booster. Caller states it could be that instead of sitting for 20 minutes they keep her longer and that when she fills out the sheet if was not an option, she knows there is severe reaction listed, she will make notes and make the person giving the dose aware that she had the delayed what they are believing to be allergic reaction and make sure, that she may never have it again and it could have been her system was so under attack. Caller states she doesn't understand vaccines and knows it's the body's job to do something and she is not reluctant helping her be immune and she understands what to do in the event and doesn't have an epi pen and her daughter has no allergies and no reactions to anything. Caller states she didn't have the worry, that there is always a chance and you worry when something foreign goes in the body that there is a chance to react. Investigations: Caller states they listened to her heart and took her vitals and tests with her fingers checking her eyes to make sure her pupils were dilating correctly and move her eyes with a moving finger. Caller states she doesn't know the medical term but the basics and let them know she was not in dire straights to rush to the hospital immediately. The patient underwent lab tests and procedures which included blood pressure measurement, body temperature: 102 fahrenheit, electrocardiogram, oxygen saturation, vital signs measurement was unknown and unspecified date. Therapeutic measures were taken as a result of lip swelling and numbness, itching on face, hands and feet / caller states then her daughters hands and feet began to itch, pressure on her chest , fever, swelling from the digging and scratching. The outcome of the events was unknown.

Five minutes after dose my daughter began to feel dizzy, felt like fainting and blurry sight. We were still at the pharmacy, I asked the pharmacist about her symptoms, about 15 minutes later, she told me she was feeling better, so we left.

Patient c/o tongue swelling approximately 10 minutes after receiving vaccination. Noticeable tongue edema, but no other sx. SPO2-98-99%, BP: 112/80 by auscultation right arm. Lungs clear to auscultation. Pt.?s mother administered 25 mg Diphenhydraamine po. No epinephrine administered. Pt observed until tongue edema diminished. No respiratory sx. Advised to contact pediatrician prior to second dose. Advised f/u with pediatrician if no further resolution of sx. Advised f/u in ED if sx worsen. Pt encounter lastsd approximately 1 hour and 10 minutes

Approximately 45 seconds after vaccination, patient began drooling and became diaphoretic, pale, and non-responsive, although she remained conscious. The patient's mother thought patient looked "blue." A Health Department nurse administered one EpiPen while another staff member called 911. Patient stopped drooling, regained normal color in her face, and began smiling almost immediately after epinephrine was administered. Nurse then took and recorded patient's blood pressure as 92/62, pulse as 64bpm, and respiratory rate as 24. Paramedics arrived approximately five minutes later and evaluated patient, who seemed to have returned to normal. Patient and her mother declined to have the patient transported to the hospital via ambulance for further observation. Patient was then driven home by her mother. Deputy COVID Operations Chief spoke with patient's mother over the phone around 6:30PM that same day to follow up with the patient. Patient's mother reported that the patient had returned to her normal self, had not experienced any further adverse events, and wished to move on from the incident.

Approximately 3 minutes after injection, our daughter was sitting next to her mother, her head lolled over the back of her chair and she began to have a seizure. The event only lasted about 45 seconds before she came out of seizing. She had a bright orange ring around her mouth, trouble breathing and vision issues. the pharmacist called an ambulance. These symptoms were mostly cleared up before medical arrived, with only dizziness, disorientation and slight vision issues remaining.

Patient states she did not eat or drink anything in the morning prior to her vaccine. Crackers and water given prior to vaccine. Vaccine administered at 0730. 0748 patient stated she was dizzy, pale, and had blurry vision. She states she felt a whoosh. She was transported to wheelchair and transferred to observation area. BP 81/34 HR 86 100% RA. Patient drank more water and crackers. Remained pale, nauseous. Denies dizziness/blurred vision. 0751 85/40, HR 68 100%RA. 0756 89/53, HR 60, 100% RA. 0801 100/49, 61, 100% RA. 0807 95/64, 57, 100% RA. Patient denies any symptoms. Educated patient and mother on when to call 911. Patient ambulated without difficulty. Discharged home with mom.

Daughter passed out after 5 minutes of receiving vaccine. She said she couldn't hear for a little bit as well.

Patient became unconscious after about 10 minutes after vaccine was given for about 10 seconds. Once consciousness was regained, patient appeared confused and pale. Patient's eyes rolled back and arms and legs were flailing about during unconscious episode

client had syncope episode after receiving 1st dose Pfizer Covid Vaccine. Client received vaccine at 1336 and was standing next to vaccine station and Father as sibling was receiving vaccine. Within 3 minutes of standing client had syncope episode and hit back of head on floor. RNs responded. PHN called 911 at 1340. Per RN client collapsed and hit back of head, Father tried to hold on but was unsuccessful. As PHN was speaking with paramedics, client regained consciousness. Client stated she remembered feeling dizzy and then woke up on the floor. Client alert and oriented, pale and stated blurry vision. RN elevated legs. Vitals: blood pressure 130/90, pulse 102, oxygen 99%. Father denied any relevant medical history, medications, allergies or history of syncope. Client stated feeling dizzy, denied pain on head or rest of body. Paramedics arrived approximately 1346 and assumed care. After assessment Father signed AMA with paramedics. Client sat in anti gravity chair after paramedics left. EMT stayed with client. Vitals at 1412: blood pressure 120/80, pulse 85, oxygen 96%. Client stated feeling dizzy and head hurts when she moves. Blurry vision resolved. PHN spoke with parents to review ER precautions and advised to make follow up appointment with primary doctor. After sibling was done with observation, client left facility at 1425 with steady gait.

Patient described that she was seeing black surrounding her after 2 minutes receiving the Covid-19 vaccination. Patient was given 10 ml of Benadryl 12.5 mg /5 ml liquid immediately. After 5 minutes taking Benadryl, patient complained that she felt that her physical condition is getting worse; she was anxious. I decided to call 911 based on her existing medical condition. The 911 team arrived within 10 minutes and checked her health condition. Patient recovered and went home safe.

10 minutes after her first dose she had been sitting in a chair & developed diaphoresis, warm skin, loss of vision,& low blood pressure. Pt was initially not responsive, when transferred to a wheel chair to take her to a gurney, left arm & left leg jerk twice. Took her to lay down and check her blood pressure. En route, in the wheel chair within about 5 to 10 seconds, she became responsive and said she could not see. Her vision returned to normal within a minute. Her initial blood pressure was 97/64 with O2 Sat 100% on room air. Her skin temp was 95.5, and her HR was 95. She continued to be responsive with excellent oxygenation and When the EMTs arrived she had a blood sugar of 90 and improving blood pressure with a regular rate and rhythm. Her diaphoresis resolved, and she continued to improve, tolerating water. Had similar reaction last year to flu shot, no hx of seizures

Patient received first dose of Pfizer at an unknown location and presented to the ED at with lip swelling and tingling. States the symptoms started 30 mins after the vaccine. Given treatment with steroids and discharged home. Patient returned to the ED on 5/15 complaining of nausea and throat tightening with a rash on her back and arms. Zofran, Benadryl, Pepcid and steroids administered. Pt was discharged home.

Parent mother reported to staff, patient experiencing throat tingling, itchiness and mild SOB while in the observation area around 0909. Per patient symptoms started approx 15-20 post vaccine administration. Patient was brought back to triage area. 0910: VS BP 110/80, HR 63, Temporal temp 98.3 F, 100% RA. Monitoring continued, MD and mother at bedside. Patient given benadryl 25mg per MD order and water. Patient reported symptoms improved after medication administration. 0920: Vitals BP 111/74 HR 57 Temporal temp 98F, 98% RA. Per patient SOB resolved. Throat itchiness and tingling improving. Leader discussed with mother regarding second dose. MD continued to monitor and patient discharged from triage at 0940 with mother per MD.

Client received the COVID vaccine (Pfizer dose 1, lot # EW0185, expiration date 5/22/2021) at 2010. The client's father accompanied her, and reported to EMT at 2022 that the client was "hearing echoes." RN, EMT, and another EMT responded at that time. Per the vaccinating RN, the client had stated that she is afraid of vaccinations and has felt dizzy with previous vaccinations, but the client deferred using the zero gravity chair previously. The client's skin was pink, warm, and dry; she was alert and oriented. She denied dizziness, shortness of breath, chest pain, nausea, or any changes in breathing. At 2026, the client's vital signs were as follows: heart rate 66, O2 98, respiratory rate 24, blood pressure 80/50 (using an adult cuff that was too large for the client). The client's pulse was normal. The client stated that "this usually happens with shots" and endorsed feeling anxious. The client was transferred to the zero gravity chair at this time and denied any dizziness with the transfer. The client's father stated that the client has no allergies or medical conditions, and that she takes no medications. The client stated that she ate lunch but no dinner today. The client was offered water to drink at this time. The father of the client deferred taking benadryl at this time. At 2030, the client stated that all her symptoms had resolved. She appeared to be sitting comfortably in the zero gravity chair and continued to drink water. Education provided regarding recommended observation time, activation and emergency management, and primary care follow up. At 2035, the client was able to stand up without any dizziness, denied any return of the "echoes" or ringing in her ears, and ambulated around the room with a steady gait. Vital signs were as follows: blood pressure 82/60 with an adult cuff, respiratory rate 22, pulse 64, O2 99. At 2040, the client reiterated that she felt "back to normal." Both the client and her father denied any further questions. The client ambulated to her car with a steady gait, and was driven home by her father.

Patient developed changes in her vision that she reported as black dots on the left side of her vision that lasted roughly 2 minutes. She said she could not see out to the left side of her eye. This went away after 2 minutes and within 20-30 minutes she developed a severe headache and was in tears from the pain. She has no history of headaches and this has never happened before. She did not report light or sound sensitivity. She did not have any vomiting, but did not want to eat. We consulted her Optometrist and Pediatrician. I gave her Naproxen and caffeine and within 3 hours her headache resolved.

Systemic: Dizziness / Lightheadness-Mild, Systemic: Fainting / Unresponsive-Mild, Systemic: Visual Changes/Disturbances-Mild

PATIENT FELT LIGHT HEADED AND DIZZY AFTER VACCINATION DURING 15 MINUTE WAIT PERIOD. MOTHER SAID HER EYES ROLLED BACK AND SHE FELL AND MAY HAVE HIT HER HEAD BECAUSE HER HEAD WAS HURTING WHEN SHE CAME TO MOMENTS LATER. SHE FELT FINE EXCEPT FOR A LITTLE HOT AFTER THE EVENT. THEY WAITED FOR ANOTHER 20 MINUTES BEFORE MAKING SURE SHE WAS OKAY AND LEFT WITH NO TREATMENT

Syncope immediately following vaccine administration. No loss of consciousness, oriented, alert, responding appropriately to questions. Described "seeing spots in front of eyes, cold feeling". Pt denied nausea.

*Per patient's mom's report* After leaving pharmacy, Pt c/o throat swelling/difficulty swallowing. Within 45 min, angioedema + tongue swelling. Benadryl + prednisone given orally @ home. Dad rushed Pt to ER, IM Epi given, Pt admitted for observation over HS.

five minutes after 13 year old patient's first dose of Pfizer COVID shot, I was alerted that pt reported that her vision was "going black." when I arrived at our designated waiting area, she was sitting down with her father. parent stated that pt has never had any type of reaction to a vaccine previously, but stated that she had severe anxiety about this vaccine. pt was moved to the floor and raised her legs onto the chair. parent declined my offer to call 911 or rescue squad. her pulse was normal. parent and patient waited a total of 30 minutes before leaving the store. pt felt back to normal before leaving the store.

Fever, eye redness, body pain starting 1 day after vaccine cough, nausea, abdominal pain 3 days after vaccine 1 episode of emesis 5 days after vaccine - Admitted to hospital with suspicion of MIS-C - As per ID and Hem/Onc, Started on IVIG and aspirin (5/23/21) - currently still admitted for management of MIS-C

Patient received her first dose of the Covid vaccine without a problem. She was reading a magazine in a chair while waiting for me (her mother) to have the second dose in the same room. About 5 to 10 minutes has passed since patient had received her injection. I looked over at patient, her eyes had rolled back and she had slumped in her chair. I ran over to her - her face was very red and she did not appear to be breathing. Her eyes were open but were not focused. I sat her up as straight as I could while the technician who administered the vaccine ran for the pharmacist. This took a few minutes. By the time the pharmacist arrived patient was gaining consciousness but began throwing up (although mainly dry retching). Her face had gone pale. The pharmacist was very helpful - she gave patient some Coke to drink and took her blood pressure, which was low. After 30 minutes of observation we were allowed to leave. The event was very frightening. Patient was very pale for the rest of the day. She had a severe pain in her left arm (and leg) which gradually improved over the next three days. We are very worried about her taking the second dose of the vaccine.

Friday, 5/21 around 11:55 she received her first dose. 20 minutes later she complained that it felt like someone punched her in the face, and her left cheek was swelling. She looked woozy, with her eyes closing, and the nurses laid her down on the floor on a mat. Her tongue and throat began swelling, and she was having extreme difficulty breathing. She then could not feel anything from her neck down, like she was paralyzed. The nurse administered an Epipen and EMS showed up minutes later. In the ambulance, EMS administered another Epipen, gave her a steroid and Benadryl and rushed her to the hospital. Once at the ER, she was still having a hard time breathing, and could not feel anything from the waist down and could not move her legs. She also complained about her spine being in extreme pain around the mid to lower back. When her symptoms did not resolve, she was admitted to the hospital for observation and treatment. She had extreme pain in her joints - spine, hips, knees, ankles and toes, if someone tried to move them, she could not move on her own. She also had tingling and numbness that would travel up and down her body - weakness in her arms and legs. The medical staff continued with Bendryl - switched to Adderax, and Motrin and steroids (Prednisone?). She was discharged from the hospital Sunday, 5/23/21 in the afternoon. This morning (Monday 5/24/21) she is still experiencing back and leg pain, weakness and cannot walk with out assistance and it is extremely painful.

Immediately after the injection child experienced a severe headache, pain at injection site, and some fatique. The severe headache has continued for 3 days. Over the counter advil has not helped. Headache is so severe that child is crying from pain. Noises above a normal tone (including the sound of a piano playing) triggers the headache and worsens it.

Almost 15 min after pt received her first dose of Pfizer vaccine, she felt light-headed. Mom who was at side held pt up and called for provider. Provider came over and pt said she had blurred vision and muffled hearing, pupils were dilated. A few seconds later, pt felt better and was able to drink water. Had pt and mother stay a few extra minutes past 15 min for observation time and patient able to slowly stand up and walk out clinic with mom. She did report not drinking water since this morning.

Initial complaint: Feeling faint (holding head), both eyes starting to go black 1046 BP 110/70 HR 60 Said "feeling better" did not eat breakfast. Drank water and ae granola bar, vision improved. 1057 BP 115/72 HR 84 She stated she was feeling better after granola bar and water.

Patient was given the Pfizer vaccine intramuscularly in her left arm. As soon as the needle was withdrawn her eyes rolled over and she slumped in her chair. We ( myself, her mother and her father) where in the process of placing her on the floor when she indicated that she wanted to get up. We placed her back into her seat and her mother proceeded to rub her back. I asked her if she needed anything but she declined. I kept her for observation for 30 minutes monitoring for any signs of shortness of breath, wheezing or swelling of the tongue or throat. No symptoms were observed. I advised her mother to notify patient's pediatrician of the incident and then released the patient.

PATIENT DID EXPERIENCE A REACTION WITIN 5MINS AFTER VACCINATION WITH PFZIER 1ST SHOT. SHE EXPERIENCED BLURRY VISION, LOSS OF HEARING AND DIZZINESS FOR 15MINS WITH HOT FLASHES. GAVE HER APPLE JUICE AND HAD HER PUT HER FEET UP ON CHAIR. AFTER 20 MINS SHE WAS FEELING FINE AGAIN AND LEFT WITH DAD.

Swollen eye; low grade fever; slight headache; fatigue; 13-year-old female patient received BNT162B2; This is a spontaneous report from a contactable consumer (patient). A 13-year-old female patient received BNT162B2 (PFIZER-BIONTECH COVID-19 VACCINE), via an unspecified route of administration, administered in the left arm on 13May2021 14:15 (batch/lot number: PFIZEREW0179) as 1st dose, single for COVID-19 immunization. Medical history included known allergies to amoxicillin. The patient's concomitant medications were not reported. The patient did not receive any other vaccines within 4 weeks prior to the COVID vaccine. No Covid prior to vaccination and was not Covid tested post vaccination. The patient experienced swollen eye, low grade fever (99.5 F), slight headache, fatigue; all on 14May2021 14:00. Treatment was received for the adverse events which included diphenhydramine hcl (BENADRYL). The outcome of the events was recovering. Information on lot/batch number was available. Follow-up attempts are completed. No further information is expected.

Present at clinic w/ dad. Dad reports prone to light headedness. Was very nervous before shot. Reported dizziness, and briefly some 'flashing lights' in vision that quickly resolved. Transitioned from lying on floor to seated to standing. Vitals stable through transitions. Gave juice box. 30 min observation post vaccine. Walked out of clinic.

Prolonged Fever (over 103), ringing in ears, dizziness, rash over entire body and face. Some soreness in legs but that went away.

fainted right away, pale, sweaty, eyes closed and then rolling back in the head. This never happened before.

Patient developed swelling in her throat and tongue within the 15 minute waiting time.. Nurses at the clinic administered an Epi-pen treatment and told us to go immediately to the emergency room. Patient was examined, monitored, and treated with three different drugs. We were at the ER for approximately two and a half hours. About four hours later she complained about similar symptoms and we returned to the ER. she was again examined, monitored, and treated again with one drug. The second visit was for approximately and hour and a half.

Patient is a 13 year old female who presents to the clinic today with her mother for a follow up ER visit from 5/30/21. Patients mother reports that patient had received her first Pfizer COVID-19 vaccination on 5/26/21, and developed a headache on Thursday. On Friday, mother and patient report that she had a very active day at school, and in Phy-Ed she did a cardio work out, followed by a 10 minute run and then golf practice after school. Patient reports that she does not frequently go running. Saturday, mom reports that patient was fatigued and rested most of the day. Sunday, she was up at 5am and was working by 6am until noon at the family restaurant. Her mother reports that it was very busy at the restaurant. Later in the afternoon on Sunday, the family went to their lake house in New Auburn, MN. Mother reports that the patient ate a good supper and the patient was asked to take the dog for a walk around 8pm. Patient took the dog for a walk for about 5 minutes, and then came back and went to her room, and her older sister found her lying in her room with her arms flexed and legs extended. Mom reports that she was drooling, staring and her muscles were stiff, and she wouldn't bear weight on her legs. This lasted approximately 10 minutes and she was transferred by ambulance to the Bloomer ER for evaluation. Initial ER workup in Bloomer showed leukocytosis at 11.4, a CK of 333, and an alk phos of 263. Her CBC, Comp, CRP, Lactate, pregnancy, lymes, COVID and UA were all within normal limits. A head CT was negative as well. Given her symptoms, she was transferred from Bloomer ER to St. Mary's in Rochester to the general pediatric floor early Monday morning. Peds neurology consulted and diagnosed her with dissociative (non-epileptic) attack, and discharged her home around 5pm on Monday evening with family. Since discharge, she reports that she is still experiencing fatigue, a mild headache, and her quadricep muscles feel weak and ache. Mom reports that she has been monitored by family since discharge and she has not had another episode since. For pain, she is taking tylenol and ibuprofen with minimal relief. She has started her menses, her onset of menarche was January, 2021, and she did not have a menstrual cycle in February, March or April, and she began her menses on 5/24 through 5/30. Clinic recommends oupatient testing for an EEG, Echo, EKG and MRI and follow up with neurology.

About 5 minutes after the shot, the patient was in the waiting area and started feeling lightheaded and dizzy. She was sweating and sounded drowsy and said she couldn't see. She did not report to be hot or itchy. We had her lie down with her feet elevated and she immediately felt better and could see again. She remained in that position for 3-5 minutes before she sat up. She felt normal again at this time.

About 5 minutes following immunization, parent came to alert pharmacist that patient was experiencing nausea and light-headedness. Pharmacist observed that patient had a drowsy appearance and her eyes would periodically roll to the back of her head. Patient was also warm to the touch. She was given a cold pack which we placed on her wrists and she said that seemed to help. Patient remained under observation for about 20 minutes before she felt comfortable to leave the store.

Shortly after vaccination patient started to feel warm and very dizzy. Said her throat felt funny but no issues breathing. Gave her some water, an ice pack for her neck. Had her return to consultation room and administered 25 mg IM benadryl. Pt felt slightly better but did another 25 mg Benadryl liquid (10 ml of 12.5/5 solution) Patient left on her own and parent was called 3 hours later and she was tired but overall doing much better and feeling fine.

Patient felt dizzy, ringing in ear, headache within receiving COVID-19 Pfizer vaccine (first dose) within 5 minutes. Ambulance arrived to take patient to hospital.

approx 25 mins after vaccination, pt developed numbness to tongue, benadryl administered. Vitals: 11:05am BP 124/80, P 90, 11:13am BP 98/60, P 82. symptoms progressed to rash, dyspnea, chest pain and pedal edema. Epinepherine 0.3 mg given with Epi pen auto injector., rescue called. 11:36am 145/70, P 118. Pt transported to hospital vis rescue

5/13 vaccination, 5/14 Woke up, her bottom lip was swollen and darkened in color; almost purplish. Her upper lip had a rash between the lip and the nose. That night, I gave her children's Benadryl. 5/15 administered benadryl again. 5/16 She seemed better but as the day wore on, her rash got worse, she complained that her lips were itchy and that they hurt. 5/17 same symptoms. Her mouth got progressively worse and sought medical attention. The dr. stated he didn't really know what was happening, if it was related to vaccine? 'just keep monitoring', keep with the zyrtec morning and night, don't delay 2nd shot. 5/20 symptoms disappeared.

Then her daughter's throat and tongue started swelling and she was having a hard time breathing. Then her daughter complained that from the neck down, she had a temporary paralysis and she could not move. Her daughter asked if she asked if she was going to die. The nurse gave her an EpiPen and called a ambulance. Her whole body was shaking, probably from the EpiPen, like almost convulsing. She said that they got her onto the ambulance and her daughter was having trouble breathing. They gave her another dose of an EpiPen in the ambulance. They also gave her daughter a steroid and some Benadryl through an IV. Her daughter was rushed to the ER, having a hard time breathing and complaining she could not feel anything from the waist down. Caller said that her daughter was also complaining of numbness and tingling all over. She also started to complain about extreme pain in her back. When they would touch her hips, toes, spine, ankles, and knees she would complain of extreme pain, like joint pain. Her daughter had numbness from waist down. If they touched her foot she could not feel it. Her daughter was admitted to the hospital and got released yesterday 23May2021. She is still experiencing pain and numbness. The numbness is from the waist down and the tingling was up and down her arms. She said that her daughter was very weak and could not feed herself. They had to give her several albuterol breathing treatments in the hospital because her chest was tight. She was not wheezing, but they could tell it was constricted and treated her with Albuterol. She said that her daughter still cannot walk without assistance this morning. She was treated with Motrin, Benadryl and Atarax, and Prednisone in the hospital. The caller said that her daughter also complained about itching all over and in her throat. The caller said that her daughter did not have a rash though.

Patient received her vaccine on 6/9/2021. after Vaccine was completed the patient was asked to sit in the lobby for 15 minutes so she could be monitored due to 1st dose. Within 5 minutes Patient stated to parent that she was thirsty, when parent grabbed a water and returned to see Patient fainting and sliding out of her chair. Went to assist patient to make sure she was OK. Patient was saying she felt sleepy and wanted to sleep. we kept her conscious but her words began to slur. Rescue came and assessed the patient who began to turn pale. She complained of her chest hurting and she started sweating. Rescue took patient to the local ER. Patient came back from ER with parent to let us know that she was OK. Her color had returned and she was no longer pale.

My daughter began having a headache a few hours after being vaccinated - that was on Sunday 6/6/21 and we didn't think much of it. The headache (and fatigue) continued Monday and Tuesday as well. But by that Tuesday afternoon 6/8, she was complaining that her vision was impaired. She couldn't see things out of the right corner of her right eye. Example, when she looked at my face she could only see the left half, not the right half. When she looked down at her phone, some numbers on the right side of the screen seemed to be missing from her field of view. Additionally, she said she was having a hard time spelling easy words (ex. "because") and was processing information really slowly when reading -- she noticed she was going much slower than normal.

At around 40 minutes he had a foreign body sensation in the pharynx and mild dysphonia. The sensation of a pharyngeal foreign body did NOT prevent breathing and it decreased as the hours passed, but it persisted for 48hrs slightly. He did not require an emergency consultation. Days later we consulted in the office of an ENT and immunology.

She was observed for 30 minutes in the center of vaccination. At around 40 minutes, she started to smell a strange odor, later a strange laryngeal foreign body sensation, her voice changed tone to hoarse (dysphonia). Shot at 4 PM Monday. Woke up at 3:30 AM Tuesday feeling very hot and thirsty. Passed out in doorway of parents room when looking for assistance, Came to and began to vomit. Vomited several times. Passed out a second time. Ambulance called. Hot, sweaty. Fever and chills, massive headache and generally uncomfortable all day Tuesday. Woke up Wednesday morning feeling 100% fine.

Got very anxious prior because of past issues with tetanus shot (similar symptoms), then felt extremely dizzy, blurred vision, momentarily fainted and then threw up about 3 times in the parking lot

The consumer reported her granddaughter received the COVID Vaccine yesterday at 12: 30. Last night her granddaughter started to break out in hives all over her body. It is on her arms, legs, belly, back, all over. She came in her room last night saying her ear was puffy and itchy. Her granddaughter is itching all over and it's bad itching. she applied Hydrocortisone Cream to the spots to help the itching and the itching is better. The hives have gotten worse. She mentioned she has recently done her grand-daughters laundry and washed her bedding/clothing in Tide. Caller contacted the patients mom and found out she normally does not use Tide. Therefore, she does not know if its from the shot itself or the change that occurred with the use of Tide. The outcome of itching all over was recovering. Outcome of other events was not recovered.

pt bled after the vaccine. I applied pressure with a cotton ball and the bleeding stopped. The patient left the counseling room and her father returned a few minutes later saying that she had turned white. I met the patient at the higi machine where she was breathing and talking but was saying she felt out of it and was thirsty. The father thinks she fainted at some point.

The patient experienced the adverse events from 15Jun2021 at 22:15 PM. The patient experienced mild seizure, could not see and low blood pressure, fainting. The patient felt cold and could not see is what she said before sitting in the chair at the pharmacy. The events resulted in doctor or other healthcare professional office/clinic visit. The patient was treated with laying down with propped up legs and water. The outcome of the events was resolved. COVID was not tested post vaccination.

Developed rash and joint swelling the next day. Also with swelling of the lips. No fever. Rash was itchy and on arms and legs. No joint swelling resolved after 48 hours. Still with pain of feet with walking on 7/9/21

Patient had a very bad allergic reaction to her 2nd dose of vaccine of pfizer on 7/6/21 . her reaction occured on 7/7/21 she broke out in bubble hives as well as her feet fingers and eyelids swelling up. today marks 4 days. I did take her to her primary on 7/8/21 he prescribed her benadryl . the next day got worse , so I took her to the hospital on 7/9/21. doctor drew a blood sample to check her organs she said everything looked ok. So doctor said she was gonna report it and I was to do so as well. Symptons have not gone away and we are going on day 4 so doctor said it should be gone in 3 days. So if shes not bring her back into hospital. I am very worried, and am up through out the night settting my alarm every 2 hrs to make sure shes not flaring up.

She woke up in pain around 6am thinking maybe she had scratched her Labia major. She went back to bed. Throughout the day the pain and discomfort grew. Around 5pm she told me that she was hurting a lot and she needed help. I examined her and it looked like a bruise about the size of a dime but long. I asked if she fell or hurt herself, no. I called the pediatrician and informed them and they suggested seeing them the following day. Took her to the ER later that night due to significant pain. They thought she might have yeast infection or herpes. Gave her some antibiotics and sent her home her pain level was a 7 and she now looked like she chemical burn. The next day I took her to pediatrician. She felt like it could be herpes or something else. Prescribed different meds and sent us home. Nothing for pain. The antibiotics worked a little, but by Monday everything was horrible. She was covered in sores from the labia major to her vulva. Took her to ER where they felt like it was herpes. She was asked about her sexual experience or being molested. They immediately admitted her to the hospital where we stayed for 4 days. With antibiotics, steroids and pain meds. She saw her Gynecologist Tuesday morning where she determined PT was suffering from aphthous ulcers, a side effect that she was aware of from the covid19 vaccine.

After about 5 minutes after getting her Pfizer dose, she was pale. She vomited and was not able to see. EMS called and when they came they checked her blood pressure which was 74/49. EMS stayed with patient for another 20 minutes until the patient became better and was okay to return home with her mom.

Immediately after the vaccination, patient felt faint, started shaking, and eyes rolled in the back of her head. Patient came to and EMS came to assess her,

13 y/o F 1st Pfizer COVID vaccine 5/13, developed left eye pain started 6/5 associated with redness, periorbital swelling, and light sensitivity. Symptoms worsened on 6/8 and able to see an Ophthalmologist who diagnosed acute anterior uveitis. Started on topical prednisone drops. Had2nd COVID vaccine on 6/14 and felt her eye symptoms weren't improving with the treatment until after 6/22. Diagnosis confirmed by two Ophthalmologists, including uveitis specialist. I am seeing her for possible Juvenile idiopathic arthritis, but this diagnosis has not been confirmed.

Pt received shot in pharmacy for second covid pfizer vaccine. She sat down for 15 minutes, but then got up and felt lightheadness. However, she continued shopping in store with her family. On the way out, she was still feeling lightheadness and her vision was lessened so she sat down at the food court area. Pharmacist was alerted to come attend to her. Pharmacist suggested that she put her head between her legs and offered wet towel for her neck. After about 5 minutes, she said she was fine to go with her family.

Medical and past drug history were reported as none. She was a healthy girl. Prior to vaccination, the patient was not diagnosed with COVID-19. The patient's concomitant medications were not reported. She did not receive any other vaccines within 4 weeks prior to the COVID vaccine. Facility where the most recent COVID-19 vaccine was administered was at a pharmacy/drug store. On 26Jun2021 at 09:00, the patient experienced ear fullness and hearing loss. There were no fever, pain, and infection. Her ear drum was deflated and red. The doctor was not sure what happened. They were not sure if it was going to go away. It has been 5 days and she has been having a hard time hearing. The events required doctor or other healthcare professional office/clinic visit. The outcome of the events was not recovered. Since the vaccination, the patient has not been tested for COVID-19. No follow-up attempts are needed; information on lot/batch number cannot be obtained.

Patient felt ill right after getting vaccine. She complained of temporarily losing vision and hearing. Patient became pale in appearance and vomited once. An ambulance was dispatched and patient was taken to an emergency room

Headache starting 10 minutes after vaccine Extreme tiredness and malaise for two days Ringing in ears Chest pain randomly since then.

- Patient received 2nd dose pfizer accompanied by parent and family member. Reported loss of hearing in one ear and reported not feeling well. - Patient ambulated to the observation area and within a few minutes, she felt nauseous. Mother assisted patient to bathroom and patient became lightheaded. - Safety officer assisted patient to wheelchair and wheeled into Special Obs area and put into supine position - Vitals taken every 5 minutes for 15 minutes. Patient was able to sit up afterwards, felt better, and blood pressure was normal before leaving. - Counseled patient to contact PCP if any further reaction or call 911 if emergency.

Patient received her first Pfizer dose at 4:06pm at 4:20 mom brought patient over to me to tell me she was feeling like her neck and leg felt swollen, we all discussed that it could be swollen lymph nodes and they choose to go and check out up front. At 4:30 Mom brought patient back to the pharmacy and said she was feeling strange. Patient said her throat felt weird, she felt dizzy and was having ear pressure, but was not having difficultly breathing. I offered Benadryl and to call 911, Mom denied her needing that at the time. While we were asking patient questions her eyes started to roll and flutter and her head would fall back, but she was alert and talking and stated that she was seeing spots. I told Mom that we were going to call 911 at that time so they could come and check her out. I also go an ice pack and we placed it on her neck, her face was flushed and I took her temperature it was 99.2 at 4:42pm. Ambulance arrived at 4:45pm and they did her vitals and she walked out to the ambulance with them to be checked out. I called Mom at 6pm to check on patient and she was doing much better and did not have to go to ER.

Shortly after vaccination the pt was very anxious and said she felt dizzy. Pt never did past out but was very dizzy and had blurred vision. We laid the pt down and elevated her feet and she was much better. Mom called EMS and the pt was monitored and recovered. I spoke with the mom Sat night and Sun and she was feeling fine. Pt returned on 6/26/21 for second dose and had no reaction.

Swollen lips after 4-6 hours Discoloration of top lip (purple in color) with redness on skin above. Started peeling 2 days after, still with purple discoloration of top lip but improved. vomiting for 2 days after - vomiting that evening. With headaches which also resolved after 2 days.

Patient reported to clinic 1+ hour post vaccination with complaints of itchiness and swelling to face. Patient denied any discomfort of the throat or tongue. Vital signs were within normal limits. Consulted with Pediatric Dr. who consented the administration of a 25 mg oral capsule of Benadryl. Patient began to feel slight discomfort swallowing. Oxygen levels remained within normal limits. Consulted with Dr. and per his assessment patient stable and no need for further intervention. Patient was observed for a total of 30 minutes following administration of medication. After 30 minutes patient reported that her symptoms were resolved.

PATIENT WAS NERVOUS ABOUT THE SHOT. AFTER THE VACCINE WAS ADMINISTERED, SHE WAS TOLD TO WAIT AROUND FOR 15 MINUTES. APPROXIMATELY 2-5 MINUTES AFTER, THE PHARMACY STAFF HEARD A THUD AND SAW SHE SLUMPED DOWN ON OUR CHAIR. THE PATIENT CLAIMED SHE IS DIZZY, NAUSEOUS, BLURRY VISION, AND MUFFLED SOUNDS. PARAMEDICS WERE CALLED AND CAME TO CHECK HER VITALS. THE PARAMEDICS CONCLUDE THAT SHE IS FINE TO GO HOME AND REST. PHARMACIST FOLLOW UP WITH THE PATIENT'S MOTHER 4 HOURS LATER AND WAS TOLD PATIENT IS FINE.

Dizziness, weakness in legs, claimed loss of vision by patient. Events occurred ~15 minutes after receiving injection. Patient transferred to emergency department for evaluation

Pt came to my office today for evaluation of itching and swelling around mouth starting 1 hour after getting her first dose of Pfizer Covid 19 vaccine. She did not have hives but rather papules. In the office her rash had resolved with the exception of mild erythema. She also complained of an itchy throat but had onset of rhinorrhea and sneezing yesterday. She took benadryl yesterday which made no difference. She had no cough, wheezing, difficulty breathing, hives, vomiting or diarrhea.

Systemic: Confusion-Medium, Systemic: Dizziness / Lightheadness-Medium, Systemic: Exhaustion / Lethargy-Medium, Systemic: Headache-Medium, Systemic: Nausea-Medium, Additional Details: patient's mother told that pt has sickle cell's disease. As rph wasn't unsure the disease's effect on covid 19 vacicne. Rph asked pt to stay in resting area for 30 minutes. Approximately, 25 minutes after the patient got the shot, her mother reported her having allergic reactions. Patient reported as having dizziness, not being able to hear, nausea, and some eye blurness. I gave water, candy, and benadryl. Also 911 called just in case. By the time emergency team arrived, pt was doing fine back

After about a minute or so, Pt. felt pretty warm/hot in the room and started saying that she could not see/vision was blurry and she started turning pale/yellow. The room was stuffy so we moved from the vaccination room to the waiting room but the symptoms (vassalage syncope) seemed to progress so we brought her into the pharmacy where there was enough room for her to lie down. We had a fan run directly on her & within a few minutes the EMTs were present and they checked her out initially & they decided to bring her to the ambulance & checked her vitals. They asked her mom if she wanted to bring her to the hospital as her vitals were all normal. Mom decided to bring her just in case to make sure everything was okay and get her checked again as a safety precaution/parental concern.

A 14-year-old female received her COVID vaccine. After about 5 minutes of sitting in the chair for her 15 minute observation, her mom said she felt nauseous and like she was going to pass out. She was laid her back in the chair. The patient's mother went to get orange juice. She was pale and her eyes were rolling back in her head. She did pass out for a few seconds, and I checked the patient's pulse and blood pressure. BP 139/58, pulse 53. By that point she was coming back around. After she sat for several minutes and had orange juice and water, she was feeling much better.

Within 10 minutes after the vaccination, there were some body tremors and dizziness, then she lost vision, hearing and ability to speak for a few seconds, followed by difficulty breathing and a feeling of her heart ?jumping out of chest.? Eventually, she recovered, but her right hand (not the injection arm) still has involuntary tremors, which has not gone away.

Approximately 5 minutes after receiving vaccination, patient reportedly became dizzy and stumbled into a floor display in the waiting area. Mother, who was with patient, states there was no loss of consciousness. Patient then sat in chair and after a few minutes stated symptoms began to resolve. Patient stated having a brief period of fuzzy vision. No signs of an allergic reaction were noted. Patient stayed in waiting area for approximately 1/2 and during that time stated symptoms had completed resolved. Patient then left the store.

So the patient complained of weird vision and her lower lip felt a bit numb. She said her normal vision came back after a couple minutes and her lips no longer numb after about ~10mins. I asked if she experienced any other symptoms or her symptoms got worsen. She answered no. Right after a few minutes of reporting the symptoms, I asked patient's mom if it's ok for me to call the paramedic. Her symptoms went away before the paramedic came. They checked on her pulse and said she's ok but still took her to monitor for further evaluation.

Starting at 0300 5/16/21 Pt was awoken with pain in R arm, R arm pit, and along back. Pain kept waking Pt up, she slept fitfully. Pt stayed in bed until around 1300 5/16/21. She had a project for school due- had to video tape making a recipe. She was standing up videotaping the ingredients when she felt very dizzy. Pt then reported that she lost her vision- everything turned black. I had to rush to her to catch her- she had loss of consciousness for about a minute. She was very pale- lips turned blue. She had a pulse the whole time, BP was 117/60- when I checked it about 5 min later. Her temperature on a ear thermometer was 99.7. After she awoke, she was still very dizzy and stayed in bed most of the day. 5/17/21 AM- she woke up for school 0600- and was feeling better. No more dizziness, color was back in her cheeks, was able to eat and left for school.

Vaccination was on Saturday, within 10 minutes of the vaccination, my child reported her ears felt like they were popping, she felt dizzy, tight throat and jaw pain. On site nurses gave her apple juice and crackers. She reported feeling unwell, dizzy, light-headed, jaw pain and short of breath the following day, on Sunday. On Monday, we took her to urgent care for same symptoms - unwell, dizzy, light-headed, jaw pain, short of breath. Rapid COVID test and strep test were negative, EKG was normal. Urgent care doctor says it is vertigo.

Patient was given her first Pfizer Covid shot in her LUA at 3:15. After the shot one of the school nurses notice during observation that she stood up and looked lie she wanted to pass out. The nurse accompanied the patient to a nearby cot where she laid her down. The patient c/o lightheadedness, dry fuzzy throat. Patient's extremities were cold to touch, eyes rolling back, hand posturing (stiff and cold), shaking., 62 pulse. No problems breathing or swollen throat. Blood pressure was 119/67 at 3:25 and pulse was 80. Nurse elevated patient legs for about 10 minutes, patient felt dizzy, shaking, apparent panicking eyes still rolling back. She was able to respond to simple commands. Still fuzzy throat. Administer 50mg IM in RUE at 3:34p.m. Blood pressure 116/71 at 3:35. Patient's face cheeks started to look red and warm. Blood pressure 123/76 at 3:39p.m. We called 911 at 3:46p.m. Blood pressure 117/78 at 3:52, temperature 97.5 degrees. Paramedics arrived 3:54p.m. Blood pressure taken by paramedics:142/86, O2 saturation 70%, started oxygen via nasal canula at 3:56. Gave epinephrine 0.5ml IM in left upper thigh. Started IV line at 4:00p.m. Red tonsils, lungs diminished but clear. 4:02p.m. transported by paramedics to Hospital.

Vasovagal Syncope @ 10:05 14 year old female. No breakfast or fluids in am before vaccination. EMT called BP 104/68 oxygen 99% HR 78 . BS 85. 10:14 seizure like behavior eyes roll back in head both arms constrict. BP 100/60 IV fluids started 100ml saline. 10:30 BP 104/70. ( Per mother history of syncope four years ago after receiving an allergy shot.) alert and oriented x4 at 10:40. released to mother .

Slight throat tightness and itchy eyes about 15 minutes after vaccination. Slight cheek warmth without rash or hives. Benadryl 50 mg PO administered with mother's consent. Symptoms did not progress and slowly resolved over a 1 hour observation time at CVC. No SOB, wheezes, tongue thickness, or hives at any time. BP, pulse, O2 sats were all normal. Initial BP - 118/78, pulse-84, O2 sat - 99%. No orthostasis at dismissal. Home with mother with precautionary instructions.

Patient states she received the COVID vaccine on 5/13/2021 at 4:00pm, on 5/14/2021 around 10:30am or 11:00am patient started to have dilated eyes, hands froze and where purple with white Raynaud's, stuttering, twitching of the left eye. Patient is still having left eye twitching but other symptoms are better.

Vomiting second day morning followed by rash on face around eyes. Rash subsiding but left eye has blood hemorrhage ( blood shot eye). 24 hrs later second eye also blood shot. Tightness of chest also started third day but now improving. No discharge from eye, no discomfort as of now.

Patient immediately became sweaty, dizzy,and experiencing hearing loss after receiving her first dose.Patient was seated on floor with elevated legs before she was wheeled back for observation. Patient was visually pale and sweaty.Patient denied any anxiousness before receiving her vaccine. Patient had vitals assessed and was given snacks and juice . Patient was reassessed after 15 minutes and reported feeling better and was sent home with parent. Patient and parent were advised to reach out to family doctor is any late onset of symptoms.

Patient received Pfizer vaccine at 09:14, Mom noticed mild Left eyelid swelling at 09:25, notified staff. No other symptoms. Walked patient and mom to the ED (next building). Upon arrival, bilateral eyelid swelling noted by this writer. No other symptoms,.

Within first five minutes sudden ringing in ears, nausea, dizziness, sweating. An urge to pass out/faint. Symptoms subsided after 10 minutes.

Systemic: Hyperventilation-Medium, Systemic: Tingling (specify: facial area, extemities)-Medium, Systemic: Visual Changes/Disturbances-Medium

a few minutes after administering the first dose of the vaccine pfizer into her left arm, the patient became very pale and couldn't hear what we were saying to her and partially lost her consciousness but then came back. when the ambulance came over, they took her out on a stretcher .

I administered the vaccine, got one side of the band-aid on and that is when her head fell back. Her eyes rolled into the back of her head; she then leaned over the right arm of the chair. Her body stiffened next. I held her head and body with the help of a colleague. This happened for about 10 seconds, then she woke up and snatched her mask off with a confused look on her face. We asked her questions to see if she was aware. She was, then she went back out falling over to the side. I called 911. When she awakened the second time after about 7-10 seconds her face turned pale. We then moved her to the floor and propped her feet up. Patient said she was nervous and felt like she had to use the restroom. Her skin was clamy; patient said clamy skin is nothing new for her. The fire truck and ambulance arrived on scene. The workers began to assess patient. Vitals were good. She did not go in the ambulance, but stated her vision were blurry. She went home with grandparents.

pt fainted/had seizure 50 to 10 minutes after covid vaccine. pt did not fall, dad held her.head and eyes rolled back, shaking and stiff for 2 seconds per dad. pt alert, breathing, and movement looked normal when I saw pt . pt mom called 911 and pt was taken to hospital . no history of fainting or seizures per dad

Approximately 2 hours after receiving her first covid vaccine, my 14 year old fainted. She was standing in our kitchen at home and became dizzy and started falling backwards. She said her vision was black, she was hot, her head hurt, I could see sweat on her face and chest, and she said she was having tightness in her chest/not able to get full breaths. She didn't totally lose consciousness, but she was disoriented. These symptoms lasted a few minutes, then she felt fine.

7 minutes after vaccination, attendee developed dizziness and a purple circle in her vision. She was assisted to a cot, lied down and her feet were elevated. HR 90 BP: 104/58 RR: 18 O2Sat: 100% Temp 98.3. Nine minutes later she was asymptomatic. HR: 90 BP: 110/50 RR: 20 O2 Sat: 100 Temp: 98.2. 10 minutes later still asymptomatic. Assisted to sitting position then vitals taken HR: 84 BP: 112/62 RR: 20 O2 Sat: 98 Temp: 98.2. She remained asymptomatic. Assisted to a standing position, Asymptomatic. Discharged to home with her mother. Able to independently walk out of the clinic.

Approximately 50 minutes after getting patient her first shot of Pfizer, she quickly complained of the following symptoms in this order: metal taste in mouth, lips burning and going numb, I can't feel my lips and then my lower lip is swelling and then less swelling in the upper lip. No additional symptoms were experienced other then surprise and worry.

8:50AM- patient started exhibiting symptoms of anaphylaxis (difficulty swallowing, difficulty breathing, lip tingling and tenderness. 9:30AM- Benadryl was administered (25mg chewable POx1) 9:30AM vital signs: HR 115, RR 22, O2Sat 98% on room air, BP 123/95, 9:33AM- EMS arrived 9:34AM- Epi pen administered . Patient tolerated well, school nurse notified. 9:45AM- Breathing appears better, still having difficulty swallowing 9:45AM vital signs: HR 120, RR 22, O2sat 95% on room air, BP 132/74 9:53AM EMS administered 10mg dexamethasone IM left deltoid, tele monitor placed on patient

Mom reported that she has had syncopal events with shots before and the student and mom thought that she had grown out of it. Student was holding her breath while given the shot. Eyes started to roll back. RN giving the shot told her to cough and RN was tapping her her back to try to get her to cough. Ice, juice, water were given to the student. Student vomited x 1, Student rested on the cot until she felt better in about 10 minutes.

Patient developed dizziness and coldness in extremities. She tried walking and fell down, but did not lose consciousness. She then alternated between feeling hot and cold and seeing "blurry spots". I noticed her lips turning white and she said she needed to lie down. I got her water and ice packs since she was sweating. We sat on the floor for about 10 minutes, then she was able to move to a chair for another 5-10minutes and then left the facility. Mom reports that she has been lethargic and napping most of the day. Patient ate a few cheez-its for breakfast and reports that she did not get a good nights sleep last night.

Approximately five minutes after the vaccination, patient became very light-headed and nauseated with loss of vision. She was offered water and hard candy while waiting for the paramedics to arrive. She took a small amount of each, but declined anything else from the store. She seemed to be in significant distress and had difficulty moving to the immunization room where she would be out of view from others in the store. Parmedics arrived and checked her vitals and said those looked ok, so they didn't seem concerned. Patient still seemed very unsteady when leaving, but did say that her vision had improved. Patients were advised to keep a very close eye on her.

On Thursday 5/27/21 her right eye swelled spontaneously while in the classroom. Both the upper lid and lower lid. No ocular involvement, or visual changes. Resolved with time and ice. Assumed that it was contact dermatitis. No other symptoms or complaints. The next morning 5/28/21 she woke up covered in hives on trunk/legs, minimal facial swelling. Lips/tongue/mucus membranes not involved. No SOB, dyspnea, cough or wheezing. No chest pain/pressure, no GI complaints or other complaints. I gave famotidine and levocetirizine. No worsening of symptoms noted and she attended school with close monitoring.

Patient received the first dose of Pfizer on 5/28/21 and during the 15 minute waiting time after the immunization, patient fainted. Patient did not fall or hit head, due to father being in the proximity and able to protect her from hitting the floor. Patient regained consciousness quickly within seconds and was moved to the floor while her feet were elevated. Patient stated that she saw a bit blurry and feeling shaky right before fainting. Patient was seen to be a bit pale in the lips but color came back after a few minutes. When patient's mother came, it was learned that the patient had a similar experience with an allergy shot in the past. Patient blood pressure was 110/71. Patient laid on the floor for a while before sitting back up.

while in the observation area the client reported that she had a feeling of her tongue swelling, face tingling , dizzy, lightheartedness , vision changing "felt strange". Able to speak full sentences, lung sounds clear, vital signs within normal limits. Calm and conversant. EMS summoned for evaluation . Evaluated and per the medic team father requested transport to Hospital .

She developed genital aphthous ulcers. They gave her pain medication in a form of Toradol and Gabapentin - helped with the pain. She soaked in a bathtub. They didn't give her steroids as they didn't want it to affect the vaccine.

On the outset: There is no proven link of the vaccine with what happened, but I am filling this just in case you see a pattern emerge: She was vaccinated (pfizer, 1st dose) on Saturday, May 15 2021. A couple of hours later she started experiencing mild symptoms (moderate headache, which we thought as a normal reaction). These persisted on and off until Wednesday, May 19th, when patient started having swollen and painful glands at the back of her mouth. Long story short, after testing first for strep, followed by blood work, patient's blood test came back positive with mono on Monday, May 24. The reason for this report is that I have read that mRNA vaccines, great as they may be, may trigger a virus response for viruses that stay within a person - like shingles or herpes etc. The way I understand it, mono also stays with a person. Patient had never been diagnosed with mono before, but mono sometimes does not have symptoms. So, is it possible that she had mono sometime before the vaccine, and it got re-triggered due to the vaccine? Again, just in case you see a pattern, I am just reporting if helpful. Patient also developed a mono rash on Wednesday, May 26; treated with topical benadryl cream.

HPI: Patient is a * year old * who presents following administration of the first dose of Pfizer COVID19 vaccine in the left deltoid. She states that immediately following administration of the vaccine, she started to feel lightheaded with ?fuzzy vision,? and pain with swallowing. She denies a history of anaphylaxis. She affirms seasonal allergies and confirms that she was feeling ?stuffy? this morning. She affirms that she ate and drank this morning without odynophagia. She relates that there was a big family argument that is causing her to feel stressed at this time as well. She affirms pleuritic chest pain, odynophagia, epigastric abdominal pain, headache, lightheadedness, and blurry vision. She denies chest pain, difficulty breathing, swelling of the face/lips/tongue/throat, skin rash/itching, cramping abdominal pain, nausea, and vomiting. Exam: GEN: Alert and oriented x 4, NAD. Patient appears to have a developmental delay secondary to eye contact avoidance, slow to respond, and speech impediment. HEAD: NCAT EYES: PERRL, EOMI ENT: Ears normal, Nose normal, OP normal, no evidence of angioedema NECK: Supple, without LAD. CV: RRR, no m/r/g PULM: Shallow, guarded breathing at rest, but clear to auscultation bilaterally with deep inspiration, no accessory muscle use ABD: Soft, no tenderness. SKIN: No rashes, skin warm and dry. No erythema or edema of the injection site. MSK: FROM, MS 5/5 NEURO: Alert and oriented x 4, CN 2-12 grossly intact, no ataxia, gait steady Clinical Impression/Field Tx: Vital signs stable and physical exam within normal limits, however, patient continued to report odynophagia, pleuritic chest pain, abdominal pain, lightheadedness, and blurry vision, even after giving patient snack/drink and reclining her with lower extremities above the level of heart and head. EMS called to the scene for further evaluation. Instructed patient and parent to consult their PCP regarding the post-vaccination reaction. Patient and parent instructed to go to emergency department should she develop chest pain, difficulty breathing, swelling of the face/lips/mouth/tongue/throat, or cramping abdominal pain with nausea/vomiting. Patient and parent demonstrated understanding of post-vaccination instructions. Medications administered: None Disposition: EMS contacted due to patient not improving. Care transferred to EMS . No workup was performed by EMS and they instructed the parent of the patient to take the patient to ER via private car for further evaluation. Father was amenable to plan and signed form.

AE: patient experienced blurred vision and ringing in the ears Treatment: No medical treatment were given. Patient was observed for 15 minutes. Outcome: Patient was discharged home with the help of mother.

Patient again reported feeling light headed and her pupils were dilated. medics took her the hospital

Patient was placed on floor with legs elevated, we called 911. after a couple minutes patient said she felt fine, so we sat her up in chair and gave her glass of water, which she drank. Patient again reported feeling light headed and her pupils were dilated. medics took her the native hospital

After administration, the patient became dizzy, shaky and sweaty. Her eyes were slowly closing and opening. I had her parent get juice and a candy bar while I monitored her. She stayed seated for at least 30 minutes and felt better after eating and drinking. She left with her parents after she felt better.

Facial swelling, left side, maxillary and mandibular areas. No pain, no redness, no increase in temperature. Mild pain on pressure on the area of swelling. No palpable lymph nodes

Patient passed out temporarily after vaccine administration. Patient was conscious again almost instantaneously (within seconds). She stated she had some blurry vision and ringing in her ears. She drank some orange juice and water. After about 15 minutes, the patient felt fine. She denied any symptoms of an allergic reaction at any point during this episode. The patient's mother was with her during this entire episode and stated that the patient has a history of passing out with vaccinations. Both the patient and her mother declined calling 911, an ambulance, or going to the emergency room.

Approximately 11:10 am on 6/7/2021 Adverse event occurred during the 15 minute wait period following the vaccination. Pt c/o SOB, weakness , diaphoresis and pale. Pt stated that she could not see and it was hard to breath. Pt stated that she has not eaten or had anything to drink today. Hx: was taken. Pt had no known medical hx or allergies to meds

Patient reported lip swelling, throat itching and a little hard to breath 30 minutes after injection. 1.5 hours later, patient developed itchy rash on arms/cheeks/neck. No intervention needed. Symptoms resolved a few hours later on their own at home. Patient did not report to MD until 6/7/21

Projectile vomiting, double vision, confusion/ delirium No treatment just gave her time. All symptoms cleared within 6 hours of onset. Approximately 9 hours after vaccine administration. Other more expected immune response followed, sore arm, headache, body aches and lethargy. Theses all resolved 36 hours after vaccine administration.

Patient stated she felt dizzy once standing after first dose of vaccine. I had her stay in the same chair she received the vaccine in for her post-vaccination wait time. She immediately felt dizzy and described her vision as pixilated. She also stated she could not hear out of her right ear. She wanted to stay seated with her eyes closed. Everything resolved within 13 minutes with no treatment.

Patient had a vagal response. She became sweaty, dizzy, her vision was impaired then she passed out or seemed to.

Patient was getting tingly, lightheaded, pale, and eyes rolling back after receiving her first dose of the Pfizer Covid vaccine. We gave her some glucose tablets and she seemed to get better afterwards. I did a pulse oximeter on her her and her pulse and oxygen was normal. I offered mother if she wanted me to call 911 and she said no. I counseled mother and the girl to monitor how she is feeling and to seek 911 or urgent care if she gets worst throughout the day. The girl left the store feeling okay.

12 hrs post - severe headache, 100.5F fever, chills. Gave advil. Worsened over night. 6/10 vomited, nausea, 101F fever despite advil, body aches, severe headache, left arm pit/axillary lymph node pain, chills, shivering, muscle pain, continued advil and tylenol alternating, severe fatigue. Temp 101.6 at 8pm 6/10. 6/11 temp 99.4, headache improved, chills improved, able to eat 6/12 discontinued advil/tylenol, mild back ache, mild pain behind eyes, otherwise returning to normal

Lightheaded, low heart rate, low blood pressure, blacking out with no vision the first hour of the vaccine. Could not walk without being held.

Patient was coming in from practice on the field. She was anxious about getting her vaccine. Per the pharmacists, the patient did not eat other than a granola bar around 1 pm for lunch. Upon receiving the vaccine, patient's eyes rolled back and patient fainted. Patient regained conscious after a few minutes and was alert and oriented. EMS was declined

On sunday night, about 36 hours after vaccination, she started to have numbness of her lower jaw and tongue. She started to have an incredible amount of drooling and could not swallow. She then started to have epigastric cramping and sharp abdominal pain that moved into the right lower quadrant. She had pretty severe headache that started the same time as numbness and drooling with photosensitivity. No fever. No vomiting but she was nauseous. she went to the ER late on 6/13. She was then transferred to the hospital for furthur care. She had a neurology consult that thought perhaps her headache, numbness and drooling could have been an atypical migraine-this resolved as of 6/14. Her abdominal pain resolved after appendectomy and now she just has muscular pain related to surgery.

Patient received her second Pfizer covid vaccine and when waiting she lost her vision and became very pale. Her eyes were open and her pupils were at maximum dilation. She could answer question in a very slow and robotic pace. She said later it seemed to her she had ear muffs on. The EMT peaople arrived and she was treated and then taken to the local hospital for observation and to see a doctor.

A 14-year-old female patient received second dose of BNT162B2 (PFIZER-BIONTECH COVID-19 VACCINE, Formulation: solution for injection, Batch/Lot number was not reported), via an unspecified route of administration on an unspecified date(at the age of 14 years old) as 2nd dose, single dose for COVID-19 immunisation. The patient medical history and concomitant medication were not reported. The patient historical vaccine included first dose of BNT162B2 (PFIZER-BIONTECH COVID-19 VACCINE, Formulation: solution for injection, Batch/Lot number was not reported) via an unspecified route of administration on an unspecified date as 1st dose, single dose for COVID-19 immunisation and patient had no reaction on previous exposure to Vaccine. On an unspecified date, the patient experienced lip and face swelling 7 days post second dose of Pfizer COVID-19. Event was occurred after use of product. The outcome of the event was unknown. No follow-up attempts are possible; information about lot/batch number cannot be obtained.

14 yo female was anxious prior to and during vaccination given at 11:59am. The patient was told to sit for 30 mins due to an egg allergy. After waiting 30 minutes, the mother (who was shopping) retrieved child who was sitting in the pharmacy waiting area post vaccination. After 5 more mins, another customer got the RPH attention explaining the patient was passing out. The mother assisted patient to the pharmacy waiting area, in which the patient was given chairs to sit on and put feet up. Another shopper who said she was an ER nurse told the pharmacy staff to have the patient lay down across the chairs then left. The patient was weak, cold, clammy, and pale. The patient then told the pharmacist that she had left her mother at the check out section, went upstairs, used the bathroom, was coming down the stairs when her vision started to have black spots. She reached her mother when she said she didn't feel good and started to pass out. The mother stated she caught her daughter. At this point, the RPH initiated a 911 call. She called out for the technician to call the store managers for assistance and take over the 911 call. The RPh then stayed with the patient and the mother. The tech gave the mother a bottle of water for the pt. The store managers arrived at the scene and also assisted. The patient stated she was feeling a lot better after she laid down for a little bit. The RPh notice color coming back into the patient's lips and face. The patient stated that the adverse effects didn't start until she was walking down the stairs. After 10-15 mins, the Paramedics arrived and further assisted the patient and her mother. The paramedics took the patient via stretcher to the ambulance for further treatment around 12:55pm.

Approximately 10 minutes after patient had received her 1st COVID-19 Vaccine she states she started to see spots and was feeling weak/dizzy and she proceeded to pass out. Family members and Pharmacist guided patient into a chair where she did not respond to verbal commands, patient was breathing normally and eyes remained open. Patient came to after approximately 10 seconds and was shaken up/scared, but overall feeling ok. Patient stayed in facility and saw provider, noted that BP was low at 88/40mmHg. Patient also had blood sugar checked which was 101 and BP was rechecked and was 90/50mmHg. Patient reported she had not ate all day and was nervous about receiving a shot. Provider determined that patient had returned to baseline and she was ok to go home.

This 14 year old female patient presented with her mother for the first dose of her COVID vaccine. She opted to receive Pfizer. Her mother and her both expressed fear of needles. Administration of the vaccine was uneventful, but during 15 minute follow up, the patient fainted in front of the pharmacy. Her eyes appeared to be moving a lot left and right. There was no shortness of breath. Her mother stated at this time that her daughter did not eat anything that morning prior to administration. Patient's state stabilized very quickly, within a few minutes, after elevating feet and lying down. Mother stated she had history of seizures.

Patient is a 14 yo girl with PMH significant for migraines who presented with one day of chest pain and nausea that woke her from sleep in the setting of multiple days of malaise. She states that after receiving the COVID-19 vaccine three days prior to admission she has not been feeling well. She reports subjective fevers and chills the day after receiving the vaccine, and intermittent feelings of her heart racing. She also has been feeling hot and generally unwell. + shortness of breath. The night of admission, she awoke from sleep with burning central anterior chest pain and nausea. She tried to stand up and felt generally weak, and then had tunnel vision and felt she was going to faint. Per mom, she was pale and diaphoretic at that time. The pre-syncopal symptoms resolved within about 10 minutes and the chest pain persisted for approx one hour. Parents called EMS and she was brought to the ED for evaluation. Of note, she reports a history of pre-syncopal sx however has never lost consciousness before. Also reports a history of intermittent palpitations. No hx chest pain prior to this episode. + fevers/chills as mentioned after receiving COVID vaccine three days PTA but no infectious sx prior to that. No cough/URI sx, no abdominal pain, no nausea. + diarrhea night of admission. No sick contacts. She is active in dance and has good exercise tolerance for that; does not spend much time outdoors. No known insect or tick bites. + FHx of sister with heart murmur as a baby that spontaneously resolved; unclear diagnosis. In the ED BP 94/62, T 97.4, P 84, R 16, SpO2 100% on RA EKG: sinus rhythm, no ST or PR changes CMP: unremarkable CBC: WBC 11.8, Hgb 11.5 Trop T 0.33 Bedside echo w/ reported good function UA wnl COVID neg CXR PA and lat unremarkable On arrival to the PCICU she reported feeling at her baseline and denied chest pain and shortness of breath.

6/14/2021-6/17/2021 1. Spots on body, itchy. (ER 6/15/21 am - Hives, not poison ivy) 2. Both hands hurt, swollen, red, very painful, hard to bend fingers or hold things. Left hand worst. 3. Feet swollen, left heal hurt for a while 4. Both wrists hurt, left wrist worst. 5. Red swollen streak down spine. Swollen 1/2" or more raised off spine. 6. Left face swollen. Right jaw hurts so bad that it was hard to open mouth. *** Took benedryl 25mg every 6 hours, zrytec twice daily, continue normal pepcid pill. Everything cleared up 6/18/21 7am. Showed daily pictures to Dr. at follow up ER appt on 6/18/21.

Shortly after the vaccine, she lost color in her face, she said her vision went blurry, then she vomited. Just a few minutes after vomiting, she said she felt fine. She waited around with her grandmother for another 20 minutes, then left, feeling well.

Patient had her first dose of Pfizer and after she sat down she became nauseous, head pounded, very hot and sweaty, coughing, her lips, face and arms got tingly. She said her heartbeat was fast and she felt like she could faint. EMT was called but they did not take her to the hospital just suggested her parent get her checked out.

Patient received vaccine at approx 9am and around 2:15 pm she started developing itchy eyes, swelling of eyelids L>R, redness of cheeks. No shortness of breath, no difficulty swallowing, no hoarseness of voice.

Pt had Pfizer covid vac this morning at 10:25 am. She tolerated injection well, waited 15 mins and left w/o symptoms. Her uncle drove her back to clinic at approx 11:13 am with pt stating she was SOB, throat felt like it was closing and her lips felt numb. Epi autoinjector pen 0.3mg was administered at 11:14 am and RN arrived on scene at that time and VS were taken: 134/89-104-99%. Pt verbalized less difficulty breathing, and no longer feeling throat closing. RN and Dr. arrived and MD provided assessment. Pt was taken to treatment room via w/c and placed on table. Pt was monitored and transported to ER when EMS arrived.

Shortly after vaccination (~15-20 minutes) patient became very faint and nauseous. Was pale in the face and vomited. Patient was seen by several firefighters that happened to be in the store at the time. She mentioned to them having fuzzy hearing, heavy arms, and sweating. They checked vitals and gave oxygen supply. Patient was then taken via ambulance to hospital but was feeling much better.

5 minutes after vaccination monitoring client had blurred vision, diaphoretic X@ altered pressure in ear, and alter mental status. Had eaten lunch- chick-fila and had no medical HX Vitals taken twice P48, BP 100/50, R16 O2 96% EKG sinus bradia, Near Syncopal P53, BP 88/51, R 20, O2 98%

complained of seeing floaters and having headaches within days of 2nd dose. Optometrist visit revealed optic nerve swelling and hemorrhaging behind eyes.

Patient was very nervous about receiving the vaccine and has a fear of needles, dad confirmed and was with patient the whole time. About 15 minutes later, patient was complaining of a tingling left hand and arm, and tingling tongue. She said she could breath without any issues, but was feeling light headed. 911 was called with moms and dads consent. With moms consent, patient got one dose of benadryl 25mg tablet. Paramedics came and took the patient.

Patient stated she felt nauseous about 5 minutes after shot was administered. She stated that she got tunnel vision, could not feel her hands. A few minutes later she stated that she felt vibrations in her hands, felt like she could not move her hands. She was visibly pale and clammy. Patient was given an ice pack and 25 mg dose of oral Benadryl liquid. Her symptoms resolved within 25 minutes of onset and she was able to leave the pharmacy feeling back to normal

Patient temporarily lost consciousness twice becoming incontinent after the second episode. Patient complained of lightheadedness, dry mouth and was pale throughout. EMT arrived and administered an IV ultimately transporting the patient to the hospital as they remained weak and unable to maintain balance.

Patient is a 15 year old female who has completed a 30-minute observation period with the following signs and symptoms of an adverse reaction: other - itching throat and tonsil swelling. Pt instructed to remove mask and breath deeply, cool pack applied to back of neck. After several minutes patient reported vision retuning to normal. Patient incontinent during episode. Pt transferred to private room. Water, peanuts, and fruit snacks provided. Sister very tearful and anxious. Water and snacks provided. Family remained 30 minutes. Verbalized feeling okay, reported slight headache. Mother made appointment for patient to see pcp today, family left clinic.

Patient went to sit 15 minutes for recovery post-vaccine with her mom. All of a sudden she said "My arm feels funny," then laid her head on her mom. Her mom said her eyes rolled back and she passed out and slid down the chair onto the floor. 911 was called, pharmacist ran to scene. Patient remained on floor, started puking. Told her to breath deep, offered water. She slowly started feeling better. Paramedics arrived and examined her. After about 30 minutes, patient left with mom. No ER needed.

When come in the counseling room for vaccinate, patient was scare and nervous. Dad had to hold her hand to calm her a little. About ten or fifteen minutes after the shot, she complaint that her vision was blurry and that she cannot see. She also complaint of nausea and about to pass out. Patient seem to get anxiety attack in a moment. I stay to monitor patient for about 10 minutes. Dad believed she got anxiety over the shot that's why she experience all this symptoms. Patient sat another 5-10 minutes and was resumed to normal

Patient began to feel lightheaded, experienced blurred vision, clammy skin, eyes dilated, 10 minutes following IM dose of Pfizer vaccine, about 2pm. Dose was administered at 1:45pm Mother asked pharmacist to call 911 because her daughter was feeling dizzy. Medical first responders arrived at 2:08pm, took her blood pressure, O2, and blood sugar, temperature. All were within normal ranges. O2=100, BP 114/60mmHg . Even though all vitals were normal, first responders recommended patient should follow up with primary care provider. First responders told mother response was likely due to nervousness or anxiety about new vaccine.

Tongue swelled up and her throat felt tight, itchy, and closed up. I have a dose of Benadryl within minutes, it didn?t seem to help. I gave a second dose of Benadryl and increased the amount and that helped. I also called the nurse line.

Pt states had "tunnel vision and feeling of everything fading", ringing of ears and sweating immediately after receiving vaccine. Pt layed down for a few minutes and felt better. No LOC. Refused transport to ER. Father and family who were present took pt home.

Patient experienced difficulty breathing and felt throat was closing within 3 minutes after administration of vaccine. Also complained of change in vision ( temporary loss of vision) and skin tone changed in tone ( face turned pale). Epi-pen was administered and 10 ml of liquid Benadryl.

Patient received her covid vaccine, while waiting for 15 minutes to be observed the patient fainted. Her mom noticed her eyes rolled back and started calling for help. We called 911 . Patient was placed on the ground carefully as we awaited for EMS to arrive. The patient was breathing, but had low blood pressure when EMS arrived. EMS took the patient to the hospital.

15 year old female found in supine position with feet elevated in vaccination area. Pt had a left upper lip injury (non-bleeding) , redness and slight swelling to left lateral eye area. She also had a less than dime sized abrasion to right knee. Pt was recovered from dizziness (syncope) and then placed into a wheelchair and placed onto a cot in the observation area with her feet elevated with a chair. VS 107/73 66 24 and an O2 sat of 99% on RA Pt timid and somewhat afraid of passing out again. Chair was later removed and then pt sat up in bed with apprehension. She ate a bag of doritos and drank a half bottle of water ( about 250 ml) She then stood up and denied any dizziness but insisted on sitting and/or lying down. She did sit back down for another 10 minutes and later stood up , denies any symptoms. Pt then ambulated to BR without help, MAEW, NAD, steady gait. She was a + o x 3, VS at 1431 111/69 74 22 with O2 sat of 99% on RA. Pt and mother given directions to change positions slowly allowing 30-60 seconds per position change. They both acknowledged this. Pt discharged home with sister and mother with direction to f/u with pcp as needed and to increase sodium and water intake today and tomorrow. No medical history Denies taking any medications Denies any allergies (name), site supervisor aware of situation. Vaccination Site

Dizzy, displayed pupil, heart rate, pale with blue around eyes and fingers. No epi pen given as patient wasn?t too short of breath and came out of it quickly. 911 was called

In her eyes, patient felt tingling and fluttering; we could not see her eyes moving at all. She also felt tingling in both arms, both hands, her face, and torso; she also had a lot of pain in her back. She felt that her face was hot but to the touch it was not. After about 20 minutes of these sensations her fingers cramped in gnarled positions that she was unable to straighten. Her right hand fingers was more rigid than her left. This lasted about 30 minutes and then she was slowly able to straighten her fingers. By the time she went to bed, around 10 pm, she felt basically back to normal.

First symptom was dizziness and nausea. Then, immediately began sweating profusely . Then began seeing spots that distorted her vision. Immediate pain in the ears, which felt as is the ears were very swollen and inflamed. It was difficult to talk because talking caused pain in the ears. Severe headache. Obviously her anxiety level escalated. We immediately found wet paper towels to place on her forehead and back of the neck to cool her. I found a hair tie and tied her hair up off her neck to keep her as cool as possible. She was sweating so much she soaked her t-shirt and was pouring sweat down her face, neck and body. We also moved patient from the chair, and placed her on the floor of the pharmacy, leaning against the wall with her legs stretched out. As a type 1 diabetic, the obvious concern was blood sugar during this reaction. Patient wears an insulin pump and Dexcom continuous glucose monitor. I was able to read her blood sugar levels during all the reaction time. Her blood sugar level spiked up. It then began to drop after a few minutes. I had an apple juice, and gave her the 15 carb apple juice.

Pt received vaccine with mother present at 1419, c/o dizziness, lightheadedness, blurry vision, difficulty hearing, at 1439. Pt states she did not eat lunch. Nursing team intervened by taking vitals, placing wet towel on forehead, lying pt in supine position with legs elevated. Pt VS stable, pulse in 80s, BP(120s/80s), 99% on RA, RR 16-18, no respiratory distress, no s/sx anaphylaxis. Pt states she did not eat lunch. Gatorade offered to pt. Dizziness improves in supine position but returns upon sitting. At 1532 pt states she has facial numbness. EMS called at 1535. EMS arrived at 1557 and assessed pt. Pt?s BG is 93, VS stable. Pt reports feeling dazed, unable to focus, blurry vision and difficulty hearing. At 1602 EMS informed pt that they will take her to Medical Center, pt immediately got agitated and started crying and hyperventilating. 1604 pt stated she had difficulty breathing and a light localized rash began to appear on her chest. 1605 EMS transported pt. Pt?s mother to follow.

Per Dr. 1.Did the patient have an immediate allergic reaction of any severity such as urticaria, angioedema, respiratory distress, or anaphylaxis (<4 hours following administration of the COVID vaccine)? Yes. 2, Did the patient previously have an immediate allergic reaction of any severity to polysorbate or polyethylene glycol? No. 3, Has the patient had prior anaphylactic reactions from another vaccine, medication or any other cause? Yes. 4, Did the patient have an allergic reaction >4 hours after administration? OR have an expected side effect of the COVID vaccine? No. Pt received COVID vaccine, Pfizer 1st dose. Approximate 5 minutes in the observation area pt complained of headache, throat discomfort, and stomach upset. She has a history of food additive allergy (dough additive), has an epi pen but has never needed to use it per mother at bedside. Initial VS: T 98.2 P 84 BP 119/70 RR 20 SpO2 99% on room air. She was monitored in the adverse reactions area. She was given Benadryl 25 mg oral x 2, and cetirizine 10 mg oral x 1. During observation period she had observed worsening oropharynx swelling, and stridor/inspiratory wheezing. VS prior to transfer was P 78 BP 102/69 RR 18 SpO2 100% on room air. EMS was called and pt transferred to ER for further monitoring, Tryptase level, and epinephrine if needed. Client vaccinated, said it was good then slumped in chair, eyes rolled back. Responded to verbal commands, opened eyes ,pupils dilated, diaphoretic, took a minute for her eyes to come into focus. Paramedic to cubicle, client transported to treatment area. Observed by paramedic for 45 minutes, departed for home with Mom and brother.

Received her first dose of the Pfizer COVID-19 vaccine on Sunday May 16,2021 at around 2:50 pm. At around 7:20 pm on May 16,2021 she stood up to walk to the bathroom. She began feeling nauseous when she stood up. While she was washing her hands, after going to the bathroom, she began to have difficulty with her vision. She describes white and blue dots obstructing her vision in both eyes. She also began to experience loud tinnitus. She described hearing multiple frequencies. The problem with her vision and the tinnitus continued as she walked through our house to a couch where she laid down. The vision problem and tinnitus continued while she laid on the couch. Her breathing was rapid but she thinks this may have been due to the stress caused by her other symptoms. This episode lasted for several minutes and then the vision problem resolved. The tinnitus continued for several minutes after the vision problem resolved. The intensity of the tinnitus slowly decreased until it finally stopped. The following evening, she experienced tinnitus for a few minutes but it was less intense than it was the previous evening. She very rarely has experienced tinnitus in the past. She has very infrequently experienced brief episodes of orthostatic hypotension when she was dehydrated. When this event occurred, she felt like she was well hydrated. This event lasted much longer and she has never experienced this problem with her vision in the past.

Suddenly my vision got super bright and I could hardly see, and then I couldn?t hold myself up and I slowly collapsed onto the ground. It only lasted a few minutes, and then I was perfectly fine. It came on very suddenly and didn?t last long, but it was extreme during those few minutes. It was also very hot that day. And I had a mild migraine headache in the morning, but that wasn?t very bad, and I get migraines fairly regularly.

Vommiting and blood pressure on the low 96/64

Patient received c19 vaccine Pfizer - lot EW0178 at 1437 on 5/26/021 At 1443, patient was given 0.15mg Epi in left thigh due to reaction. BP 104/68 P: 63 O2: 97% RA (laying) BP 101/70 P: 88 O2: 98% RA (laying) Patient was seen with her head back in her chair, then sliding down the chair with her eyes rolled and unable to breath. RN assisted Patient mother at side Lab Mgr present Patient was kept on the floor until 911 arrived for EMS transport to Hospital.

About 5 minutes after receiving her vaccine, patient described feeling light headed, had hearing and visual disturbances, felt like she might pass out. after several minutes of sitting with head between her leg, those symptoms went away but she said she had developed a headache. At time she left pharmacy she said she felt ok other than her headache.

Patient claimed she saw spots and was then unresponsive to the mother. Blood pressure was taken prior to EMT arrival with blood pressure of 105/68 and pulse of 70. Patient barely ate that day prior to coming in for the vaccine. Mother later disclosed that the patient had a event similar to this prior (date unknown) with the patient after getting an IV injection.

ABOUT 30 SECONDS AFTER GETTING THE COVID VACCINE, PATIENT BEGAN FEELING DIZZY AND GETTING BLURRED VISION. I HAD HER SIT DOWN AND TAKE A DEEP BREATH AND GAVE SOME WATER, THEN SHE BEGAN FEELING A LITTLE BETTER AND ABOUT 30 MINUTES LATER SHE RECOVERED AND LEFT THE PHARMACY WITH HER FATHER. I CALLED THEM ABOUT 2 HOURS LATER AND CHECKED IN AND HER FATHER TOLD ME SHE IS FEELING BETTER NOW.

Moments after the vaccine administration, pt started to experience difficulty breathing, lightheadedness, flushing of the face, swelling of lips. Epinephrine 0.3 mg IM was administered, 911 contacted. A few minutes after the epinephrine administration, pt started to breath better. EMT came in within 5-10 minutes. Checked pt's vitals, administered required care and took the pt to the ER.

Immediately after the patient received the vaccine, the patient turned pale and said, "I feel a little dizzy and my vision is blurry." The patient fainted and ended up having a seizure. Once the patient regained consciousness, the patient was nauseated and vomited. Minutes later she had another seizure but immediately came out of it. We asked the patient if this happen to her during the first dose and she said it did not. The patient does not have a history of epilepsy. The patient's mother did say she had the exact same reaction when she had braces put on. The patient sat in the waiting room for 30 minutes while drinking water. Before the patient left, we had check up on her and she said she felt completely fine. I (the pharmacist) called the mother a couple of hours later to check up on the patient. The mother said she felt fine as if nothing ever happened. I did recommend to her that her daughter gets checked up at urgent care today just to be on the safe side. The mother said she has an appointment with her primary care next week.

On 5/30/21, patient woke up around 10.30 am and complained of severe headache in back of head (pain level 8-9/10). She was nauseous, unable to sit and even stand. She was getting rainbow circles in her vision. Rushed her to emergency to UCSF, drs ran various blood tests and brain MRI but everything was normal. It was also found that Bipasha had high BP 149/61. The drs could not make out what happened and they discharged her later on after giving some IV fluids and some pain medicine. On 5/31 and 6/1, at home, patient was seemingly okay. On 6/2, Wednesday, patient developed headache again, same symptoms like on 5/30. We rushed her to Urgent care in Fremont and saw her pediatrician dr and a pediatric neurologist. Her BP was high then also 144/87. No cause could be found why. The pediatric neurologist was suggesting it is possible that some Inflammation could be causing the blood fluctuations. Orthostatic testing resulted in major fluctuations of BP. Both neurologist and neurosurgeon have ruled out hydrocephalus. They are saying heart inflammation could be a cause.

Immediately got a shooting pain in arm with light headness. Got pale within 30 seconds. Started to have a seizure with uncontrollable arm and leg movement. Arms went limp broke out in a clammy cold sweat. Pupil got enlarged and were unresponsive. Patient blacked out and this continued off and on for 15-20 minutes post injection. Had very labored breathing. EMS arrived and attended to patient

Pharmacist notified & went to help pt in the post vaccination area. Pt was diaphoretic, pale, and appeared as though she was going to faint. Pts mother, pharmacist, and bystander assisted patient to the ground with legs bent, water, crackers, and ice pack provided. Patient was starting to become more responsive. Paramedics called.

Patient stated that she has lightheaded and ringing in her ear. Patient was helped to lie down on the ground with both feet elevated. After 20 minutes, patient was feeling better. Blood pressure at this moment was 101/69, and pulse was 74. Patient was able to stand up and walk around.

Systemic: Fainting / Unresponsive-Medium, Systemic: Flushed / Sweating-Medium, Additional Details: Immediately after I gave her the injection I looked at her face and her eyes rolled up and her pupils were dilated and I said to her Are You OK and that was when her head fell back and and it hit the back of the wall. I looked at the top of her head and it was red and she was sweating. Patient was still sitting and we called 911. Paramedics came and checked her out. She became aware and responsive within minutes. She stayed about 30 min.

Patient received 2nd dose of Pfizer vaccine today. The patient's mother asked for assistance as daughter was "having a reaction." Vitals/assessment at this time: 62, 16, 82/54, 99% on room air. Patient looks pale, diaphoretic, says "my vision is blurry". Questioned mother and patient, mother said "this is the first time this has happened." When asked about if she had anything to eat, she said she had not eaten breakfast. OJ given. Patient awake, alert, oriented X3. Sitting in chair. Skin warm to touch/ looks flushed. Vitals Reassessment- 72, 92/59, 95% room air. Repeat vitals taken x 2. Patient noted feeling better. Patient drank all orange juice, moving extremities, talking, walking with steady gait. Home with parent.

After getting vaccine, pt states "I can't hardly hear, I can't hardly see" to the primary nurse. Provider was sent for, and pt was placed in supine position; pt then reported anytime she saw blood or needles, she felt lightheaded. VS 1524: 91/58; 60; 16; 96% on room air. Pt awake, alert, oriented to person/place/time/situation the entire visit. During VS check, pt reports she is feeling better, back to "normal." Pt placed in upright position and given an apple juice to drink. Pt also reports she hasn't had a thing to eat/drink today. Repeat VS 1535 94/67; 74; 16; 95% on room air. Pt then ambulated to waiting area where she was observed for an additional 5 minute by provider.

Pain at injection site, sore throat, fever, nasal congestion, runny nose, lose of taste and smell, shortness of breath, high level blood clotting factor, pneumonia. Doctor visit on 5/26/21 OSF Virtual visit 5/27/21 , prescriptions prescribed consisted of: Montelukast , Levocetrizine DHCL , Azelastine HCI Nasal Spray , Fluticasone spray (was not administered because insurance would not allow scripts to be filled until 5/28-5/29/21). ER visit on 5/27/21 Treatment on 5/27/21-5/28/21 consisted of steroid via IV, Inhaler, antibiotic Prescriptions prescribed: Albuterol Sulfate , Azithromycin, Prednisone

Feeling nausea, HOH after injection she stated. Same s/s after 1st injection she stated. @16:34 BP=71/38 Pulse=67 O2=97% @16:38 BP=76/47 Pulse=64 O2=97% @16:43 BP=90/61 Pulse=67 O2=98 Pt.presented with dizziness and hypotension. Mother refused IV fluid therapy stating "she's ok, she's just being dramatic." Mother signed refusal and released.

Patient experienced an anxiety/panic attack after vaccine administered. Patient stated her vision was blurry and that she was "okay". Patients mom was beside her during this time and shared that this happened a month ago while giving blood. During this time patient was able to state her name but still appeared fatigued. Patients vital signs were taken at 1425, Bp: 110/70, HR: 65, O2: 99%. The patient was directed to focus on breathing in which she performed easily and stated that her vision was now clear again. Patient was also offered water in which she drank with no problem. At 1455 her vitals were taken again, Bp: 128/84, HR: 75, O2: 96%. Patient and her mom stayed for an additional 15 minutes for close observation. Patient fully recovered.

Aproximately 5 minutes after receipt of 2nd dose, patient began feeling hot, and dizzy. Symptoms progressed to gagging, loss of vision and hearing. Patient was placed in the modified tredelenburg postion, vital signs obtained and monitored for roughly 40 minutes. Vital signs were initially low, RN assessed patient had not even prior to vaccine adminsitration. Offered water and energy bar, placed bag of ice on back of neck. Patient did not experience and respiratory distress throughout event. Mother was educated on signs and symptosm to monitor for post vaccine and if any new or worsending symptoms presented to seek care at the emergency room.

The evening of the vaccine Maddison had slightly labored breathing, pins and needles feelings in feet and hands. Fever, body aches. The morning after while taking their temperature, Maddison said she couldn't see anything then fell into my arms. Her pupils quickly dilated, she made gutteral sounds while shaking slightly then lost consciousness. I couldn't see her breathing for about 15 seconds and she was confused when she came to. She couldn't hear, see or speak during the episode. We took her to the emergency room where a chest xray and ekg came back clear.

itching all over, even in her ears; feeling short of breath; Chest hurting; itching all over, even in her ears; skin was red from scratching it, but thought it had been somewhat blotchy, too, on her legs; This is a spontaneous report from a contactable consumer (patient's grandmother/Guardian). A 15-years-old female patient received BNT162B2 (PFIZER BIONTECH COVID-19 mRNA VACCINE, Solution for injection, Batch/Lot number: EW0167, Expiration date was not reported), via an unspecified route of administration, administered in left arm on 19May2021 at 16:30 hours as 1st dose, single dose (at the age of 15-years-old) for COVID-19 immunization. Medical history included seasonal allergy and allergy to cats (from an unknown date and unknown if ongoing). Patient had no covid prior vaccination. Concomitant medication included clonidine extended release (ER) at a dose of 0.1 mg daily taken for an unspecified indication, start and stop date were not reported. Patient was not covid tested post vaccination. 0n 19May2021 at 19:45 hours (about 3 hours after receiving her shot), patient complained of itching all over, even in her ears, patient also mentioned her chest hurting and feeling short of breath. Patient was not sure whether her skin was red from scratching it, but thought it had been somewhat blotchy, too, on her legs, possibly because that was the area uncovered so she could see it. The blotchiness and chest pain resolved quickly, but the itchiness continued. Patient also complained of stomach pain, which occurred after patient's grandmother gave her a loratadine. The loratadine settled the itching down, but patient reported nausea the next morning and shortness of breath, as well. Patient received Claritin 24 Hr for the events as treatment. The outcome of the skin was red from scratching it, but thought it had been somewhat blotchy, too, on her legs and chest hurting was recovered on May-2021 and recovered for rest of the events in 2021 Follow up needed, further information has been requested.

Pt. fainted during the 15 minute post-vaccination waiting period pt. was hypotensive, with blurred vision, and upon being placed in a supine position recovered within 30 seconds.

Approximately 30 seconds after injection of covid vaccine patient stopped talking, her eyes rolled back and she slumped over in chair. Patients head rolled side to side and patients mom started freaking out thinking she was having a seizure. She screamed at the technician who administered the vaccine to call 911 . Patient regained consciousness after 10 seconds and was fully alert and able to speak/ answer questions. Pt maintained consciousness the whole time while waiting for the ambulance. Pt's mom still wanted an ambulance so one was called and EMT's came and accessed the patients vitals. Vitals were all within normal limits and EMTs spoke with mom and advised that pt most likely did not have a seizure and gave the option to transport to the hospital or monitor at home. Mom chose to monitor at home and pt was able to walk out of the store on her own.

Pt reports she had her first dose of Pfizer Covid vaccine on 5/17/21. The following week she had a very heavy period that lasted longer than usual with severe cramps and occasional lightheaded/dizzy spells. After her cycle ended, she continued having brief episodes of dizziness, usually when going from sitting to standing, usually resolved quickly (after a few minutes) with rest. Over the past two weeks, the episodes have become more frequent and intense, and over the past few days episodes also include s/s of seeing spots and having blurred vision with nausea, SOB, and palpitations/tachycardia. Tonight while at lacrosse practice, pt suddenly became dizzy and felt like she might pass out, became diaphoretic and pale.

Approx. 5 minutes after vaccine given Pt. felt weak and like she was going to pass out. She was sitting in chair and we gave her water and she put her head between her legs. After approx. 2-3 minutes she felt better and sat up drank water. After about 2 more minutes she felt dizzy and she went limp, she was diaphretic, pulse 50, bp 80/palp. Pt. arroused and was able to speak approx. 2 minutes later but was very weak and states "I cannot see anything". Squad was called they arrived and assumed care. Pt. was alert and oriented and was able to talk. Pt. Mom was with the patient the whole time

she had low grade fever, chills, headache, but next day she complain of pain on the left side of her body (not just the arm) I was told this was normal and to give her Tylenol or ibu. she has swelling in her left axilla. on Sunday she notice some blister on the inner corner of her mouth. I have messaged her PCP with no response

idiopathic intracranial hypertension with blurry and double vision secondary to cranial nerve 6 palsy

A 15-years-old non pregnant female patient received BNT162B2 (PFIZER-BIONTECH COVID-19 mRNA VACCINE), dose 1 via an unspecified route of administration, administered in Arm Right on 15May2021 16:00 (Batch/Lot Number: EW0182) as single dose for COVID-19 immunisation. Medical history included no other illness. There were no concomitant medications. The patient did not receive any other vaccines within 4 weeks prior to the COVID vaccine and any other medications within 2 weeks of vaccination. Prior to vaccination, the patient was not diagnosed with COVID-19. Since the vaccination, the patient has not been tested for COVID-19. The patient previously received Amoxicillin, Azithromicin, Cefdinir, Gardasil(Pf) and had allergy. The patient had Nausea that does not go away with Gardasil. She also received HPV vaccine previously and experienced nausea and the dizziness for a month. Reporter stated, "she did not have any allergic 'condition' (not clear) but she was allergic, she had always had a rash when she had this Amoxicillin this antibiotic. She was allergic to Penicillin." The patient had the COVID Pfizer Vaccine on 15May2021 and as soon as she had her first dose she started to feel nausea and dizziness and from there 5 days later she had, she felt a terrible headache, 'this heaviness' (not clarified), headache, dizziness, palpitation. The reporter took her to the emergency room, they did all the test and they didn't really see anything in the results. From there she cannot stop to have nausea and dizziness and clogged throat so the reporter had to give allergy medication to her every day. She did have, she never had an allergic reaction to any vaccine but the HPV vaccine (captured as unspecified vaccine in tab) what was similar to what she is experiencing now. She had nausea and the dizziness for a month that she couldn't stay in school and she has this nausea and dizziness and clogged throat she has to run for this allergy medication what normally she never needed. So, she doesn't want to take this second dose and the reporter was wondering if it is Pfizer because she was just like (Height), a (Weight) pounds if because the reporter understand the vaccine is one for everybody, isn't this strong for a kid with that height that weight, that you do not needed the second dose. The patient could not she said she will not because she goes to school and she feel the nausea and dizziness and so. The reporter would like to report it because it maybe Pfizer's that she was thinking that 1 dose would be enough for kids. When asked about the start of experiencing adverse events, the reported stated that "Well the nausea and dizziness, she was a little bit not sure about like headache right after like her we stayed there like half hours because the doctor said to stay there because she has her reaction to which but then we just the reporter felt the nausea and everything was so bright you know, the reporter had the same effect but her went away and the patient after we received she was, the whole day but not too bad, 5 days after then she but she was already with this nausea, dizziness and 5 days after she got the palpitation, bad headache, nausea, dizziness and then I took her to the emergency room." Reporter stated, "No they have improved, the only thing now left is every day she has this nausea, dizziness and then she said about dizziness and then nausea and then clogged throat and then the reporter gave her the 'allergic pill I gave that, to this allergy' (not clarified further) really she gave only 1 tablet a day." Reporter stated, "On 20May2021, they did a blood test, they did urine test, they did a ECG, so the hospital did all and the doctor too, nothing new." The reporter had lab test result on chart, she stated they put on her chart but she also had the copy of the results. Okay, here they put the result is a line on the chart. Okay here they put like linear line lab test complete, she get all the line because here they didn't put results but screen for metabolic and kidney disorder they did, they did an 'urinary tract infection' (not clarified) they have marked in the ECG. Okay you going to ask for specific (incomplete sentence)." The patient had no other illness but reporter need to know what Gardasil and Covid 19 vaccine have in common because the symptoms were very similar. Nausea that does not go away. She asked if it is the preservative or what ingredients. Never had a reaction to any other vaccine. She just wanted to call the Medical Department if she should have the vaccine on Saturday or not. On 15May2021, 4:30 AM, the patient experienced dizziness, nausea, lightheadedness, blurred vision, headache, a little palpitation (as reported). The patient received treatment Children allergy medication. Adverse events resulted in Doctor or other healthcare professional office/clinic visit and Emergency room/department or urgent care. Outcome of the events clogged throat, heaviness was unknown and other events was not recovered.

Adverse event: Patient felt dizzy, nauseated, blurry vision 5 minutes after vaccine administration. Treatment: Patient was suggested to lay down and it helped resolve the symptoms. Juice and crackers were offered to the patient as well. Patient was feeling better a few minutes after food and rest. Patient was discharged to POV/Home at 1:25pm.

A 15 yrs old female patient complained she could not see anything 10 minutes after receiving Pfizer vaccine. She seemed sleepy,and her face looked pale.She was responsive and conscious, She denied difficulty of breathing, She was hot to touch and asked for water. After approximately 5 minutes, her vision was back to normal,and her face color changed to its normal complexion. the patient has not received anything during this period. She was encouraged to be calm and breathe in and out slowly.Vitals not recorded. EMS arrived and patient went to the ER for further follow-up. Patient's mom confirmed patient remains stable an hour after the incident.

Upon leaving the store approximately 15 minutes after receiving vaccination, patient told her father that she felt she was having an issue with her vision and was feeling faint. Her father was able to support her as she fainted and was lowered gently to the ground in front of the store. A pharmacy technician was leaving the store simultaneously and upon witnessing the event ran back to the pharmacy to tell me I was needed to assist. When I arrived, the patient was alert and breathing and able to answer questions regarding personal information, date, time and location. Patient stated she felt normal other than some tingling sensation in her hands. Patient reported that she hadn't eaten anything since waking up that morning. EMS was already en route, and I observed the patient until they arrived. Patient has since been transported to the hospital and is currently reported in stable condition.

IN ER VISIT: history from patient and mother: Immediate onset lightheadedness, dry throat, intermittent neck and leg pain, then blurred vision. In ER rapid twitching of eyelids and rapid eye movements, not consistent with nystagmus. Exam otherwise normal, except lying in chair with eyes closed, answers appropriate but brief. Abated with ativan and zofran. Seen at 1912 PM in ER, came by private vehicle with mother. Discharged from ER at 2300 alert ambulatory and improved.

Experienced a headache that has not went away. She is very fatigue, chills, enlarged lymph nodes behind her ears and under armpits, dizzy spells, sensitive to light.

Complaints of seeing stars and feeling faint. Wheel chair and stretcher pad with legs up. Mom states she passed out with needle sticks or the sight of blood. 94/62, pulse 92, resp 14. Passed out a for few seconds. Mom declined EM. Pretty sweaty. 97/65, 92/14 states she feels better. Given water and cool cloth. 10:45 am 91/63, 97/14. Patient states no longer feels faint. Setting up . 11:00 Patient states she feels normal. Skin warm, dry and pink. Standing vital signs 116/86, 16. Mom and patient comfortable and walks without problems. Instructed to notify staff prior to vaccine so she can lay done before administration.

This is her 2nd medical event within the month of vaccine, previous VAERS reported Seizure activity appearance on 6/27/21 witnessed by parents. Started with yawn facial expression, drooling, pale, unconscious, shaking, called 911, normal appearing once they came, brought to facility and normal eval and EEG

The patient medical history was not reported. The patient received birth control medications within 2 weeks of vaccination. The patient did not receive any other vaccines within 4 weeks prior to the COVID-19 vaccine. Prior to vaccination, the patient was not diagnosed with COVID-19. Since the vaccination, the patient had tested for COVID-19. There were no known allergies. On 12Jun2021 16:00, the patient experienced Full body rash, full body itchiness, swelling of the skin, toes, fingers, and lips. Swelling goes up at night and down after waking up, Joint pain from minimal movement. The events led to emergency room visit and hospitalization for 2 days. The patient underwent lab tests and procedures which included nasal swab and results were negative on 13Jun2021. Treatment received included benadryl, motrin and atakin, no imaging or steroid medication given. The outcome for the events was not recovered. Information on Lot/Batch number has been requested.

Swollen bottom lip (left side only); This is a spontaneous report from a contactable consumer. A 15-year-old female patient received bnt162b2 (PFIZER-BIONTECH COVID-19 VACCINE, solution for injection), dose 2 via an unspecified route of administration, administered in Arm Left on 04Jun2021 16:30 (Batch/Lot Number: EW0178) as 2ND DOSE, SINGLE for covid-19 immunisation. The patient's medical history was not reported. The patient received first dose of vaccine BNT162B2 on 14May2021 (at 15 years) for COVID-19 immunization (product=COVID 19, brand=Pfizer, lot number=EW0182, administration time=16:00 PM, dose number=1, vaccine location=Left arm). Concomitant medications included tretinoin (ALTRENO) taken for an unspecified indication, start and stop date were not reported. On 05Jun2021 21:45, the patient experienced swollen bottom lip (left side only). Therapeutic measures were taken as a result of swollen bottom lip (left side only) AE treatment: Benadryl (after call to dr). The outcome of the event was unknown. Shortly after reporting this she fainted and her eyes went back into her head. I ran to her and helped her mother lower her to the ground. I noticed her eyes were now open but she was pale and listless. I called to a tech to call 911. I asked her if she could speak and she nodded. I asked if she had eaten and she shook her head no and her mother said she had not.

Lip swelling over the 1st day after shot. Then persistent hives which have not resolved for nearly 1 month.

Patient got vaccinated by the pharmacist at 09:57 am and while waiting for 15 minutes at around 10:05am patient felt dizzy and patient told family member that she felt dizzy and having bad test. patient also told family member she have hard time of hearing what the family member was saying to the patient. family member ask pharmacist for help and pharmacist called 911 and provided patient water. After 5 minutes EMT personal showed up onsite and took over the incident.

Around 9 AM, my daughter came downstairs and said her legs felt heavy. She sat down and then she said she was really hot. She was also sweating and pale. She said that she felt like she was about to pass out and that she couldn't see. She also said she couldn't focus, that she was seeing stars, and everything looked black. My husband and I told her to lay down on the couch. We gave her a rag to put on her head to cool her down and we got her some water to drink. She said she had a headache and we gave her some Tylenol.

This consumer reported for a 15-year-old female patient (reporter's daughter) And This is the first case of 2 split cases. A 15-year-old female adolescent patient received a dose of BNT162B2 (PFIZER-BIONTECH COVID-19 VACCINE, Formulation: Solution for injection, Lot Number: ER8729, Expiration date: 31July2021), via unspecified route, administered in right arm on 26Jun2021 09:30 am(at the age of 15-year-old) as DOSE NUMBER UNKNOWN, SINGLE for COVID-19 immunization. The patient medical history and concomitant medications were not reported. On 26Jun2021, after the vaccination in the night, the patient experienced Fever, Nauseous, she was sick to her stomach and she was afraid she would throw up. And on 27Jun2021 the patient experienced Blurry vision, lost her hearing briefly for few minutes this morning, no treatment options were used. Reported said that this is kind of a different side effect, she did read that someone had blurred vision, but she said she also lost her hearing. The event deafness assessed the as serious. The outcome of the events blurred vision was unknown, pyrexia and nausea are not recovered and deafness is recovered in 2021. No follow-up attempts possible. No further information expected.

Attendee received first dose Pfizer vaccine at 15:08 in the left arm. Lot# FA6780. Attendee was directed to observation area. At 15:10 fainted in her chair x 20 seconds, her parents were by her side and she did not fall out of the chair. When she aroused she reported feeling lightheaded, dizzy, mild nausea and ring in the ears. Attendee's skin was cool, clammy, and pale. Cot brought to the patient area. Attendee assisted to the cot to lie down, legs elevated. Icepack applied to her neck. 15:13 HR: 88 BP: 108/58 RR: 14 O2Sat%: 97 Temp: 97.0

Visions went completely white. Ringing in ears. Clammy sweaty skin. Partially incoherent for 5-10 seconds.

Began having tightness in the throat and difficulty getting a deep breath within 14 minutes of receiving the vaccine. Voice became hoarse. Later lips began to swell. IM Benadryl and epinephrine were given. Then she was taken to the Emergency Room. Symptoms continued and she was given Solumedrol.

Client was vaccinated and waiting in the observation area when she stated she was seeing black spots. Client was taken to the reconstitution area in a wheelchair where her vitals were taken and she was asked to lay in a supine position. Parent explained that client gets like this or has similar experience if she does blood draw. Client was asked if she had a meal and she said she ate very little and had done her morning exercise. Client declined having her sugar done. Client was hydrating with water then her vitals were taken again while sitting up. Client parent stated that her BP is usually normal. Client was asked if she wanted to go to an emergency room. Client responded that she was feeling better and wanted to leave with her mother.

Patient received vaccine at (13:22) directly afterwards, patient states "I feel hot, my eyes are turning black, I cannot see." "I am sweating bad." Patient was noted to be pale and offered wheel chair. Patient was taken to private room and assisted to lie down at (13:30). Patient continued to be oriented (BP - 118/40) HR-76 / O2 -97. Patient complains of dizziness and black spots in vision. Continued to monitor. Cool rag applied to forehead (1:34 PM - BP -123/69 HR - 75 / O2-98) (1:36) (13:37) Patient assisted to sitting position. BP -114/59 Voiced feeling better. (13:40 PM) BP - 120 /75 / (13:40 - stood patient up and assisted to chair. Patient was normal and fine while sitting in chair. (13:50) Patient was clammy, stated she felt as though she would pass out. BP - 119/27 / HR - 89 O2 98 / assisted to lie down, continued to complain of dizziness. Clammy, (13:59) EMS called / BP -115/92 - HR -76. Ambulance arrived at (14:05). Before transfer of care BP - 125/71 - HR 107 / O2- 98

She started with a fever 102, chills, headache, body pain. As of today she still exp sinus pressure, ear pain both sides (side going down gland to neck), headache and body pain. She was taken to the doctor informed to give Tylenol and Ibuprofen.

A 15-years-old (non-pregnant) female patient received BNT162B2 (PFIZER-COVID-19 VACCINE, Batch/Lot number and Expiration date was not reported), dose 1 via an unspecified route of administration, administered in Arm Left on 11Jul2021 13:30 (at the age of 15-years-old) as dose 1, single for covid-19 immunization.Medical history included urinary tract infection from an unknown date and unknown if ongoing. Concomitant medication(s) included cefalexin taken for urinary tract infection, start and stop date were not reported.The patient previously took azithromycin for drug hypersensitivity and experienced rash, azithromycin for drug hypersensitivity and experienced swelling. On 13Jul2021 at 06:00 the patient experienced rash and swelling on face, feet/toes, arms, fingers, and a little on back, tonsils slightly swollen, joint swelling, swelling on arms, fingers. Adverse event resulted in patient visited Emergency room, department or urgent care. patient received treatment with Zyrtec.The patient did not tested for covid before and prior to vaccination. The patient did not receive any other vaccines within 4 weeks prior to the COVID vaccine. Device Date: 14Jul2021.The outcome of the events was recovering. Patient sitting for prescribed observation time. I noticed her blinking rapidly and stepped out to ask if she was ok, she stated her vision was blurred and then proceeded to have what appeared to be a very brief seizure (5-10 seconds in length) then passed out briefly (10-15 seconds) when she came to she seemed completely normal again. We had her lie down on the bench and she drank some water, she was sweating more than normal. After 30 minutes or so she sat up for 5 minutes or so then stood and walked with mom out of store. Mom called pediatrician to see if they could have her seen.
