Patient described feeling thirsty, then threw up immediately after. he reported feeling better afterward.

Pt reported sweating & nausea. BP 101/74; HR 93; RR 24. 30 min post vaccination. Pt feeling much better. Advised pt to get rest, rehydrate & what to do in case of emergency. Released in stable condition

Patient received the vaccine. Patient got very dizzy shortly thereafter. He started vomiting and slumped his head over in the trash can. BP was 98/58 with pulse of 56. Oxygen was 99. We got him to a hospital bed. 5 minutes later his bp was 102/60 with HR of 68. Pulse was now 79 and oxygen 99. 10 minutes out, his bp was 101/65 with a pulse of 72.

Vomited Pulse 80 No other sign or symotom of ditress. Adult present. Informed to seek medical attention with any furthter problems.

The patient received his first dose of the COVID vaccine. The father states as they were walking out, the pt complained or "not feeling well". He describes this as feeling of dizziness and nauseaous. The father asked is he could come back to an urgent care room to lay down which is where he had a n episode of emesis.

Pt had vasovagal reaction and emesis immediately post vaccine admin. NO ALLERGY to vaccine. Okay to receive 2nd dose. At 1:37PM, 6 min post receiving vaccine, patient told mom that he felt lightheaded and was having pain at injection site left arm. Rn assessed immediately and pt reported feeling "sick". He drank few sips of water. MD called to bedside. I assessed pt at 1:39PM, pt sitting chair, elevated his legs onto the footrests of wheelchair sitting in front of pt. +Facial and lip pallor, and complaining of nausea followed by 1 episode NBNB emesis consisting of what he ate for lunch 1 hour prior. Pt immediately reported feeling better post emesis. Lips appeared pink. Pt brought to triage area, lay supine on stretcher with legs elevated. Nausea and "lightheaded" sensation resolved prior to lying down. BP improved post emesis from 75/39 to 84/46 at 1:48 PM. Stayed lying down for 5 minutes after which time pt wanted to sit up. Upon sitting BP and HR increased 96/64 and 69, stable with no nausea or lightheadedness, only remaining sx "slightly weak". drank 12 oz apple juice and ate 2 graham crackers without any further emesis or symptoms. Observed for 15 more minutes and was ambulating without return of symptoms, discharged home with mom and brother with instructions to seek medical care if symptoms return.

12 yo male presenting with bilateral lower extremity paresthesias and confusion. Admitted to the hospital. Pending discharge to rehab facility. ED Note 5/19/21 Patient is a 12-year-old male with hypogammaglobulinemia, asthma, IBS who presents with weakness in his lower extremities. 6 days ago, patient had seafood and later that night vomited once, nonbloody, nonbilious. 4 days ago, he received the first dose of the Pfizer SARS-CoV2 vaccination. That day, he also felt some burning over the anterior parts of his bilateral ankles. Over the past 3 days, he has been sleeping a lot and had a headache. He has not had a fever or other muscle aches, no subsequent N/V, abdominal pain. Last night, he reported ankle pain to his mom. Today at school, he was playing kickball when he had the sensation that his ankles were not working/gave out and he fell to the ground. He had difficulty getting up and required a wheelchair. He did not lose consciousness, denied CP, palpitations, SOB, headache, vision change during or before that episode. He was not incontinent of urine/stool and had no abnormal movements noted at the time. Since then he has noted twitching in his thighs, calves and toes. He reports weakness throughout both lower extremities and has difficulty with intentional movements at the toes/ankles/knees, weakness in hips. He reports a burning sensation over the anterior ankles/dorsum of foot b/l. Denies fevers, chills, cough, SOB, CP, palpitations, abdominal pain, N/V/C. He endorses diarrhea 3x daily which is his baseline. He has panic attacks 3x daily w palpitations/SOB that self resolve. He has been on keflex for an ingrown toe nail for the last week. MRI performed with normal brain and spine. However, on repeat exam patient has diminished sensation in the bilateral feet and diminished ability to dorsiflex or plantarflex both ankles with very limited toe range of motion. Still with preserved reflexes in the patella and Achilles. Discussed at length with neurology and the family. Differential diagnosis at this point of functional neurological disorder versus Guillaine Barré syndrome. Neurology note 5/21/21 Patient is a 12yo M with hypogammaglobinemia, IBS, and poorly controlled anxiety, presenting with 4 days of lower extremity paresthesias and 1 day of hyperacute onset of lower extremity weakness and perhaps some mild confusion. History notable for covid vaccine three days prior to presentation and anixety surrounding covid and return to school. His general examination is notable for anxious affect with difficult to localize neurologic exam. Exam shows possible bilateral lower extremity weakness of TA and hamstring>quad/IP that is very challenging to grade due to poor effort and giveway, and decreased sensation in a bilateral stocking/glove distribution to all sensory modalities. MRI w/wo contrast brain and spinal cord normal on admission. LP perfromed in ED also normal. Given negative workup and history of anxiety with recent psychosocial stressors, most likely sudden onset weakness is secondary to a functional neurologic disorder. Plan for discharge to rehab pending PT evaluation.

5 days prior to admission on 5/16/21, he received a first Pfizer COVID vaccination. The day after the vaccine, he had neck pain and stiffness. He subsequently developed fever, diarrhea, and vomiting. The day prior to admission, he developed a diffuse popular rash. He was seen in clinic on 5/20 and lab work was obtained, which was consistent with MIS-C findings (elevated troponin, elevated fibrinogen, elevated CRP, hyponatremia, relatively low absolute lymphocyte count).

Patient was immunized with Pfizer vaccine as a minor with consent from mother. Patient had no issues initially with vaccine. Following 15 minutes after administration, patient noted flank pain and both sides of abdomen and also labored breathing. We called a code white in the store had management as well as pharmacist offer ambulance to patient and also monitored the patient. Mother and son left AMA and went to call patient's doctor.

Pt recieved vaccination and at post vaccination waiting station, pt felt nauseated and a little dizzy. I raised patient's feet, gave him some water and placed a cold ice pack on his neck. He then threw up multiple times. After about 15 minutes, pt felt a lot better and left with mom

Pt c/o nausea and dizziness. BP 104/37, HR 53, RR 14. Pt stable and released from the vaccination site.

Nausea, headache 5 mins post vaccination, minimal epigastric tenderness No shortness of breath, no rash Patient drank water, no vomiting. Patient felt better, no more abdominal pain. Patient craved for fried chicken.

During patients 15 minute wait he had a syncopal episode and fell out of his chair. Once I got to the patient he had come to, mom said he was out for about 10-15 seconds max and patient says he did hit his head. Oxygen saturation was 99% on RA, heart rate was 62bpm. BP 110/78. Patient was alert and oriented once I was assessing him. He was able to sit up in the floor about 5 minutes after coming to. He was able to drink water and the PCT had placed cool wet paper towels on his head and neck area. We provided the patient crackers and the management team was available. The patient said he felt like his stomach was upset and did not want the crackers. I suggested he go to the ER and mom said she would drive him and asked if this was caused by the vaccine itself. I explained what could have occurred and educated them on where he hit his head he needs to go to the ER to be assessed. Mom agreed and the manager got a wheelchair from the front of the store and mom drove him to the ER. I called report to ER.

AE: patient felt nauseous 15 minutes post vaccine administration and vomited, felt faint but did not pass out. Treatment: no medication was given. patient was under observation. Water improved symptoms Outcome: symptoms resolved and patient was sent to POV/Home.

Inflammation of arm where injected. A lot of arm pain day after vaccine. Little nausea. Very tired. Night sweats.

PT mother states that the next day her son started with fever 24 hours after the vaccine. He had a fever for the next 8 days. At times his fever got up to 103F. His other symptoms were that his lymph nodes in his neck got very swollen and painful to touch and was very tired. He also threw up 3 times on various days. He also had a very mild sore throat before his vaccine. His pediatrician thinks it was the vaccine with in combination with his virus, was tested for strep, mono and covid, and all was negative, also lab work was performed and all was negative. He slowly started to feel better after his fever went down.

Next day and every day thereafter he started getting more symptoms of feeling sick. The arm pain began the next day after injection. Followed by runny nose, sore throat, croup sounding cough, shortness of breath, gasping, chest pains, vomiting, back of the skull pain. He described the head pain as a square from the base of the back of his skull to the crown of his head and back down. Felt more like a pressure in the bones of his skull and less like a standard headache. The chest pains are sharp and come on sporadically. At first they were accompanied by hacking cough but then progressed to sharp chest pains at random times especially when lying down. He has difficulty sleeping and staying asleep. Also, lower abdominal pain but no vowel issues.

Patient nauseous, vomited and anxious. Patient vitals-BP(120/70), HR(81), Resp 16. Vitals are stable and patient will be transferred via ambulance to hospital with mom.

Patient had initial fatigue/ somnolence in AM day after vaccine, in PM developing fever, HA, sore throat, myalgias stomach ache and chest pain. Chest pain every 20-30 minutes, worse when supine, pain with breathing when laying down that resolved when sitting up, presented to ED for workup. Found to be tachycardia to 100s, hemodynamically stable otherwise. Pro-BNP 193, CRP 1.57, Total bilirubin 1.1, d-dimer 274, troponin initially normal then elevated to peak 3.45, CK normal, ESR normal, CBC normal, BMP normal. Chest x-ray normal. ECG normal sinus rhythm, diffuse ST changes and J-point elevation in inferolateral leads. Echo obtained read as normal, with EF 60% without pericardial effusion. Working Diagnosis pericarditis/myocarditis. Cardiology and infectious disease consulted, and started on ibuprofen, tylenol, IVIG, methylprednisolone. Labs for COVID antibodies, lyme, echovirus, HSV 6, EBV, CMV, Parvovirus B19, coxsackie, adenovirus pending. Cardiac MRI planned to be completed.

Sore arm at the injection site; whole body and muscle aches; fever of 101 degrees; whole body feels warm; nausea; fatigue and sleeping more than usual. Symptoms started at 4 AM the following morning, gave him some Tylenol to bring down the fever, and have him drinking lots of ice water. He has no appetite because he feels nauseous but he was able to eat a piece of bread which seemed to settle his stomach somewhat.

Diarrhea beginning later in the day after receiving the shot. Reports about 5 episodes a day, one sibling with similar symptoms 3 days after. Neither ate anything unusual I tried to reassure mom but she is convineed that the sx are due to the vaccine.

Patient reported feeling nauseous and vomited a few minutes after receiving his second dose of pfizer. Pt looked pale and reported feeling lightheaded. Pt did not show any signs of respiratory distress: no swelling of the throat, face or mouth observed. Vital signs were within limit: BP 125/72, HR 60, RR 12 (at 3:45pm), BP 120/74, HR 62, RR 13 (at 3:45). Patient reported feeling better after vomiting. Observation nurse took vital signs, offered water and Gatorade. Pt left our site at 3:50 (Patient's mother was driving).

Severe vomiting and diarrhea, & dry heaves, which ruptured blood vessels under eyes & bruising under eyes for over 12 hours. Luckily it let up before having to take him in to the Emergency Room for treatment to stop the vomiting & diarrhea.

My son had a fever, seizure and vomiting. We administered valtoco (diazepam nasal spray) for seizure.

Patient stated he was feeling nauseous and had a headache. Patient was given snacks and vitals were taken by EMS. No known allergies. No past medical history. BP: 104/67. Pulse: 95. Os Sat: 98. Patient started feeling better and refused to go to the hospital by EMS.

Approximately 26 hours after receiving injection (around 6:30 pm on a Friday), he began to vomit and have extreme fatigue. The nausea and extreme fatigue persisted through Saturday and Sunday. He was only able to keep minimal liquids and food down. On Sunday afternoon (around 1pm) we had a low grade fever. This was the first time he was noted to have a fever. After consulting with telehealth, it was determined that there may be concern about his appendix. He was admitted to the ER about 5:30pm on Sunday, and was diagnosed with a perforated appendicitis. An appendectomy was performed at 1am Monday.

Patient admitted with a diagnosis of myocarditis and encephalitis, currently still hospitalized at the time of this report. From EHR note: Patient is an otherwise healthy 12yM with recent Lyme disease (erythema migrans) transferred to hospital for hyponatremia, encephalitis, myocarditis. Initially intubated due to aspiration risk, now s/p extubation and stable on RA. Broad workup so far including consultations from neurology, cardiology, immunology, and infectious disease with unclear etiology, s/p MRI and LP with c/f covid-vaccine related myocarditis/encephalitis vs lyme carditis/meningoencephalitis vs less likely MISC vs other. Infectious testing so far negative, patient otherwise has returned to baseline behavior without any current symptoms. He is transferred to general for further workup and evaluation and close monitoring. Diagnosed with Lyme 19 days prior to admission, treated with amoxicillin; p/w headache, fever and vomiting so concern for encephalitis as above vs inflammatory process post-COVID or vaccine. Less likely meningitis given reassuring initial CSF studies. Negative COVID antibody. Resolving hyponatremia, thought to be likely SIADH. Given presentation and maternal history of albinism with immunodeficiency, broad workup sent. Also with c/f possible inflammatory response/MISC-C vs vaccine myocarditis.

high blood pressures 170/63, 165/95; He is feeling like a choking type of feeling on the base of his chest; chest discomfort; Hard for him to swallow; This is a spontaneous report from a contactable consumer (patient's mother). A 12-year-old male patient received BNT162B2 (PFIZER-BIONTECH COVID-19 VACCINE), via an unspecified route of administration in left arm on 20May2021 18:15 (Batch/Lot Number: EW0168) as 1st dose, single for COVID-19 immunisation. The patient had no relevant medical history and no concomitant medications. No additional vaccines was administered. The first day, the patient was fine but on day second (22May2021), he was feeling like a choking type of feeling on the base of his chest, like in the lower base of his throat. He said that it was hard for him to swallow. He was breathing normally. Patient's mother took his oxygen, he had enough oxygen but he says it felt weird and then he was having high blood pressure. Patient's mother wanted to know if it was normal. The patient experienced high blood pressures and chest discomfort on 22May2021. He has had blood pressures of 170/63. Today, it was 165/95. Events did not require visit to emergency room or physician office. The chest discomfort resolved on 23May2021 however the high blood pressures continue and have remained about the same. Patient's mother stated she does not want to give her son anything because she was not sure about the vaccine. Outcome of chest discomfort was recovered on 23May2021, of high blood pressure was not recovered, of remaining events was unknown.

A 12-year-old male patient received bnt162b2 (PFIZER-BIONTECH COVID-19 VACCINE, Batch/lot number: unknown and Expire Date: unknown) on 22May2021 via unspecified route of administration as 1st dose, single dose for covid-19 immunisation. As medical history it was reported that, he had allergies and allergic to a lot of things, mold, feather dust and everything and dyspepsia. He had been taking Claritin for allergies (but not when he got the vaccine). She stated her son did not have any allergies to medications. Concomitant mediations were not reported. On 26May2021, 4 days after that he was complaining of chest pain. Caller wants to know if her son should get the 2nd dose of the vaccine. Caller stated, "He had other symptoms. On Tuesday the chest pain went away then complained again on Thursday. His doctor gave him an EKG and his heart rate was fine. Caller did not take him to the ER at the time because did not want a medical bill. His blood sugar spiked up. He had sweating, vomiting, diarrhea. He said he had "sharp pain in my chest and never felt that before" His blood sugar spiked up. He was dehydrated. He had ketones from it. Caller gave him extra insulin. He had chest pain from the vaccine. It was on Tuesday and happened again on Thursday but had not come back since then. It subsided. EKG was ok." No one was informed (doctors). I took him to the regular doctor. She did not know. She was not aware of the symptoms. They are not specialized in the medications they are giving. It was hard to give them any kind of referral. Both of us had vomiting, headaches and chills. He had never had chest pains from any vaccines. He had fatigue, these are symptoms form the covid shot. It was 4 days and then 6 days after the vaccine. A fast heartbeat. HCP are not going to help. If they are dosing the covid shots but they are not given the vaccines they did not want to do anything. There was no accountability. Caller having a hard time. No one can give me an answer, no one. Not from the experts, no one was telling me. The vaccine administration should have been through the doctors, they will not take your data without understanding the vaccines. All the kids 12 and up and all male and like my son and probably a lot more but they are not saying it. Caller scared. My son did not have a heart defect. Caller did not know. Heart inflammation; they need to know if it was safe or not. It was scary to know no one knows what was going on or not. Caller asked about myocarditis. The patient had heart burn before but said the chest pain was not that. With the symptoms the caller saw online, she thinks the patient had pericarditis. The patient's next dose was on 12Jun2021.The caller was reading online that people reported chest pain and ended up in the emergency room and the caller did not want that. All the events occurred on an unspecified date in May2021. The patient underwent lab tests and procedures which included blood glucose increased: spiked up on May2021, electrocardiogram: fine on 25May2021 EKG and his heart rate was fine, heart rate increased: fast on 25May2021. The outcome of event chest pain was recovered on an unspecified date in May2021. Therapeutic measures were taken as a result of dehydrated. The outcome of blood sugar spiked up, sweating, vomiting, chills, diarrhea, felt sick, extreme fatigue, and slept on and off for about 4 days, dehydration, fast heartbeat and pericarditis was unknown. Information on Lot/Batch number was available. Additional information has been requested.

Patient was at immunization clinic for second pfizer immunization with his mother and older brother. His brother had his immunization and patient had his second immunization given reassurance and diversion for immunization offered to lay down for immunization but refused. Giving home instructions to family and patient stated felt funny, color pale, unresponsive for approximately 5 sec, lowered to floor turned on side, patient became nauseated and vomited x 1 small amount of brown emesis, Skin moist and warm to touch pale. EMS here to assist and taken back to observation area via wheelchair. Report given to EMS. Patient awake.

Medical history included migraine. The patient's concomitant medications were not reported. The patient previously received first dose of bnt162b2 (BNT162B2), via an unspecified route of administration on an unspecified date (Batch/Lot number was not reported) as single dose for covid-19 immunisation. It was reported that patient did not have fever after first dose of vaccine. On an unknown date, after second dose of vaccine administration, the patient experienced nausea, fatigue, headache and fever 100 Fahrenheit. Events occurred after the use of product. Laboratory tests and procedures included body temperature 100 Fahrenheit on an unknown date. Outcome of the events nausea, fatigue, headache and fever 100 Fahrenheit was reported as unknown at the time of this report. Information about lot/batch number has been requested.

Fevers and fatigue first 24 hours then nausea, vomiting and right lower quadrant abdominal pain. He was found to have an acute appendicitis approx 48 hours after his second covid vaccine. He received preop antibiotics and underwent a laparoscopic appendectomy.

Severe abdominal pain on 6/5/21 off and on from 8:30am through 6:30pm. Went to Hospital Emergency Department. They performed blood tests which showed some inflammation (but obviously don?t know where) so they performed an Abdominal US which was normal. Patient continued to have severe abdominal pain and they proceeded with a CT scan with and without contrast, which was also normal. They couldn?t say if it was a reaction from the vaccine but possibly, since everything was WNL.

Eyes dilated, limited vision, limited hearing then threw up. Felt better after throwing up. 12:25 pm 6/12/21

Headache starting that evening, chills, fatigue, nausea, muscle soreness/weakness, sore throat, muscle aches, red welt at vaccine site that is getting bigger by day.

Had 2nd Pfizer vaccine at 5:15pm Thursday June 3. Felt fine that evening, just complained arm was starting to hurt when he went to bed around 11pm. Threw up twice on Friday June 4 at 9:30am & 1:30 pm. Very uncomfortable, bad headache, no energy. Mostly slept from 6pm Friday June 4 to 5am Saturday June 5. Immediately threw up when he woke up Friday. Threw up again about 30 min. later and 4 more times during the day. Still having bad head pain. Started to become confused and say things that didn't make sense. Went to ER around 4:30pm Saturday June 5. Blood work showed his sodium level was dangerously low. He was immediately given sodium and transported to Hospital ICU. Had brain MRI, lumbar puncture and lots of blood work done. Brain MRI showed signs of encephalitis. Also had myocarditis. Was in hospital until Thur. June 10. Encephalitis and myocarditis seemed to resolve quickly and he was just treated with rest and fluids along with antibiotics in case there was any viral/infectious cause of symptoms. All such tests have been negative to date

Felt faint and nauseous five minutes after injection. Skin felt hot and sweaty. Said his stomach hurt and he felt like he was going to faint. Removed his jacket, sat him down, fed him crackers and water and he recovered after about five minutes.

Patient complained of nausea and dizziness after vaccine. Patient was sat down and BP was taken. Pt allowed to sit for 20 min in the flu shot room until recovered. Reaction was non-allergenic and attributed to anxiety.

Nausea, fatigue, chest pain, hand tremoring started last night and this morning suddenly 3 occupancies of chest pain followed by feeling ok Was strong pain

After receiving the second dose on 6/15, my son woke up at 3:30am on 6/16 with a nose bleed that lasted as a steady light stream for about 5 minutes. He also had a slight fever of 100. By 7:30am on 6/16 his fever was slowly increasing and the nausea began. He did not throw up but it is now 10:45am on 6/16 and his fever is 102.6 and he is still nauseous. I'm not too worried since these are all common symptoms but I felt like I should should report them since he is 12 years old.

After the shot given by (tech), within a few minutes the patient felt dizzy, blurred vision, then stomach felt irritated and patient started to throw up. At this moment when I saw the patient, he also had nose bleeding. While sitting, then patient stood up and was then asked to stay on sitting position. Soon patient stopped throwing up and right away felt much better. Within 15 minutes of observation patient return to normal and felt fine. The patient had breakfast right before the shot.

Patient had Covid vaccine yesterday morning. Developed fever and vomiting at 9pm last night. Brought to the ER poorly responsive. Found to have a large intracranial bleed with an apparent ruptured AVM. Sent to larger hospital for pediatric neurointerventional radiology and pediatric neurosurgical care.

Patient received the Pfizer vaccine on Sunday (06/13/21) and had mild upset stomach, headache and felt achey for approximately 24 hours post vaccine that then fully resolved. At approximately 4am on 06/16/21 he developed sharp substernal chest pain that was located centrally and a bit to the left with pain worse on inspiration and with movement. No other symptoms at that time. Went to the Emergency Department where he was transferred to Hospital as his troponin was elevated at 4.02 ng/ml. His chest pain resolved with 1 dose of Ibuprofen 400 mg.

A 12-years-old male patient received BNT162B2 (PFIZER-BIONTECH COVID-19 VACCINE, Solution for injection, Batch/Lot Number and Expiration Date: unknown), dose 1 via an unspecified route of administration, administered in Arm Right on 14May2021 09:00 as single dose for COVID-19 immunization. The patient medical history and concomitant medication was not reported. The patient previously had the known allergies included hives developed after 1 dose of amoxicillin and Azithromycin, hives present after several doses of m (as reported). The patient did not have the COVID prior vaccination and was not tested for COVID post vaccination. The patient did not receive other vaccine in four weeks. The patient received Multivitamin and Probiotic in two weeks. On 14May2021 at 01:00 PM, after the 4 hours of dose the patient experienced blister-like hives appeared on tops of ears, then upper back and back of hand. Then, sore arm, nausea, fatigue, tremors, headache through night. Following day continued with hives, headache, fatigue, sore arm, plus vomiting. Felt much better by the second day post shot. Medical history included covid-19 on an unknown date in Oct 2020, allergy to animal (cats) from an unknown date and unknown if ongoing. The patient's concomitant medications were not reported. Prior Vaccinations (within 4 weeks) and AE(s) following prior vaccinations was none. Patient's mother mentioned that day after his shot, he experienced fever and diarrhea (on 20May2021) which lasted for 7 days. Caller states the fever was more like 8 days and the diarrhea was at least 8 days, she would have to ask him when it completely went away. Caller states her son lost 5 pounds (on 20May2021) when he got sick on an unspecified date, he was 90 pounds currently, he was 95 when he got the vaccine. She states he kept losing weight over the week. Caller states his stool was green and he was pooping everything out, it was gross on an unspecified date. On the 2nd day, he experienced vomiting (on 21May2021). He was also tired for a long time on an unspecified date. Caller states her son had been staying at his dad's house when he was sick. She states she would've taken him to be seen earlier because after 5 days of fever, he needed an IGG drip in case it was his heart or similar to Kawasaki. She requested him to get an IgG Drip because, according to her, any child who has a fever for 7 days might have the virus that causes Kawasaki disease. This is based on her experience with her daughter who has Kawasaki disease. The son was not given the IgG drip because the doctors were not sure. They also tested his stool, but he did not have anything wrong. She mentioned that her son had COVID in October, the one that got sick for a week, then goes away, and comes back again. He was sick for a while with all the symptoms, and it was proven that he had it. Caller states when he got sick, they wanted to rule other things out, so they tested his feces for a bunch of things, but he was negative for everything. As of calling, the son had recovered but she was requesting for him to have an ultrasound of the heart. She was asking if he should get the 2nd dose. The events resulted in physician office visit. The patient underwent lab tests and procedures which included culture stool negative on an unspecified date. The outcome of the event fever, lost weight was recovered on 28May2021; vomiting recovered on 21May2021; diarrhea, fatigue recovered on an unspecified date; and the outcome was unknown for faeces discoloured and illness.

Fatigue starting approx. 12 hours after second pfizer covid vaccine shot and lasting 24 hours; reduced appetite; fever from approx 24 hours after shot (maybe sooner) until approx. 36 hours after shot. Highest recorded temp was 102.2 at approx. 30 hours after shot. Fatigue interfered with daily activities from 12 hours after shot to 36 hours after shot. Mild soreness in arm where shot was given. Pulse and blood oxygen were checked each time temp was taken (approx 3 times between 24 and 36 hours after shot) and were within normal ranges each time; although pulse was on the higher end of the normal range for a 12 year old boy. Quick recover starting 36 hours after shot, when temp reduced to approx. 100. Appetite returned at this time. Fever was gone by 48 hours after shot and all other side effect seemed to have subsided by this point. No mediation was taken by the vaccine recipient. Vaccine recipient was offered usual foods and he consumed milk and water in smaller amounts than usual but also more frequently than usual to stay hydrated.

Pt states that they have a hx of anxiety w/ vaccines. Did not take any PO intake since the AM (vaccination @ 15:05). By 1515, pt states that they are feeling better. Advised pt to take next vaccine in observation room. Pt stable and released from the vaccination site @1535 w/ father. Pt was ambulating, speaking in full sentences.

12 y.o. male patient with history of developmental delay, seizure disorder who presented to the emergency department today with acute encephalopathy, unresponsive with bradycardia, V-tach requiring defibrillation, and acute respiratory failure requiring intubation. The family had been traveling and vacationing with extended family members and recently returned Saturday night. No sick contacts during the trip and he recently received his second COVID-19 vaccine on June 5th. On Sunday, June 20th, he had cold symptoms with stuffy nose, sneezing and minimal oral intake. Mother gave him 10ml of a multi-symptom OTC cold medication that evening. Unsure what is in the medication or if it contains Benadryl. This morning, he had an 11am swim lesion and then came back and slept on the couch. Still minimal oral intake and was resting all day long but awake most of the time. This evening at 5:43pm per mother, he received another 10ml of the multi-symptom cold medication. He was given his Vimpat and Valproic Acid medications around 7:30pm. Around 8:20/8:25pm, he was asleep and had an emesis and was not responding In the ED, he was unresponsive and poor respiratory effort with episodes of apnea and was incontient of stool during the exam. He was brought to the resuscitation room, PIV placed x2. Placed on oxygen initially. Ativan given once for possible seizure. Epinephrine given for bradycardia. Noted V-tach on rhythm. Second Epi given, started Epinephrine infusion and Shock given 2J/kg. Cardiology at bedside. Rocuronium and Etomidate given for intubation. He was intubated on the second attempt with 6.0c ETT.

Chills started at 12:20 am, followed by nausea and vomiting. This lasted for 5 hours. He also had a fever of 101.7 at the highest. This lasted for approximately 24 hours. He also had fatigue and lack of appetite. He was given zofran for the nausea and tylenol for the fever.

Adverse event: It was reported that patient received first dose Pfizer vaccine at 08:37. He had emesis in post-observation at 08:40. Treatment: Assessed by post-observation RN. EMT called and assessed. Patient taken by EMT to private observation area with parent. His father stated he (the patient) did not eat before vaccination. Given juice. Patient also stated he was very anxious about being vaccinated. Outcome: Patient asymptomatic. Vital signs stable (per EMT). BP 104/70, HR 82, Resp 16, Temp. 96,4 F, O2 Sat 98%). EMT discharged patient with parent to home.

He says he woke up and couldn?t smell anything or feel textured with his fingers. He then threw up and got a headache and has a fever of 103.2.

Medical history included known allergies: Penicillins. Concomitant medications were none. No other vaccines within 4 weeks prior to the COVID vaccine. No other medications the patient received within 2 weeks of vaccination. Historical vaccine included first dose of BNT162B2 on unspecified date for COVID-19 Immunization. Patient experienced Appendicitis and appendectomy starting 5 days after 2nd shot (11Jun2021). The evet result in Doctor or other healthcare professional office/clinic visit, Emergency room/department or urgent care, Hospitalization, Life threatening illness (immediate risk of death from the event). Patient was hospitalized for 1 day. Treatment Appendectomy received. The outcome of the event was recovered in Jun2021. Prior to vaccination, the patient was not diagnosed with COVID-19. The patient previously received first dose of BNT162B2 (PFIZER-BIONTECH COVID-19 mRNA VACCINE), via an unspecified route of administration on unspecified date as a single dose for covid-19 immunisation and had no reaction. Concomitant medications included famotidine (PEPCID), vedolizumab (ENTYVIO), vitamin C and a probiotic all taken for crohns disease from an unspecified start date and ongoing. On 18Jun2021 patient experienced nausea and vomiting in morning and also had a fever, chills, headache. The outcome of the events was unknown. Pt began experiencing a short burst of nausea shortly after injection. Pt began dry heaving. Pt's skin became cool and clammy. Pt was evaluated during this time by a Physician's Assistant, as well as a Registered Nurse, and vital signs were all within normal range. Pt began feeling much better shortly after, and was able to leave the clinic after their observation period without any other medical attention.

pt complained of getting dizzy and having trouble seeing when talked to him after. patient had parent with him (father) pt was nervous. got vaccine and left room. father said he looked very pale and then pt sat down and started to throw up. patient wasn't hospitalized. mild redness at injection site. left the premises after 15 to 02 minutes feeling fine.

Pfizer vaccine. Pt has had diarrhea since Sunday (7/11) morning. He was also very tired and sleepy on Friday (7/9) evening after the vaccine.

12 Y 4 M old male with a history of chest pain that started two days ago on Monday. On 7/11 night, he didn't feel well felt nauseous. He was able to sleep but had chest pain the following morning that improved after he vomited. The chest pain is midsternal and feels pressure-like. It lasted a few hours. He swam on 7/12 without problems (at a swim club) for an hour. He had his second dose of COVID-19 vaccine on Saturday 7/10/21. There has not been any fevers. Elevated troponin and CRP with recent COVID-19 vaccine, suspicious for post-vaccine myocarditis. He is hemodynamically stable with normal vitals and no evidence of ventricular ectopy or depressed left ventricular function. His troponin peaked at 11.05 and trended down, but his most recent value is slightly up at 8.2 from 7.6. Recommended continuing to trend the troponins for now (if they show a downward trend then twice daily is adequate, unless he develops chest pain). At this time, IVIG is held unless he becomes unstable or the troponins trend upward .

He presented as pale, shaky, and almost ready to faint. Patient was given pedialyte, ice, and chocolate after vomiting one time. EMS was contacted, they arrived and assessed the patient. He was cognizant and able to stand on his own. Mother refused transport to the hospital and the family left about 45 min after immunization was given.

? At approximately 6:50PM vaccine was administered. ? Within 10 minutes of the dose being administered, patient began vomiting. ? At approximately 7:03PM, RN attempted to assess patients pulse. RN determined pulse to be approximately 100 BPM but was difficult to be confident as the pulse was indistinct. Another RN also attempted to collect a pulse and was not confident in a reading. Oxygenation and heart rate was attempted to be collected using a pulse oximeter. A reading could not be determined. Additionally, a temperature could not be collected using an infrared thermometer. Upon observation, the patient appeared pale and diaphoretic. He did not present or report difficulty breathing at any time. Patients skin was cool to the touch. Patient reported to his parents that he felt nauseous and cold. ? At approximately 7:08PM patient was offered juice. ? At approximately 7:10PM RN observed that nail bed began to become discolored, slightly blue. RN requested call 911. ? EMS onsite by 7:15PM. EMS vitals included, o BP 131/88 o HR 103 o Resp. 24 o O2 Sat 98% o Temp 98.4 ? Family declined glucose screen and transport to emergency department as patietn began to report feeling better. Patients color improved. ? EMS, family, and staff left scene at approximately 7:50PM. ? RN connected with patients father on 7/16/21. Father reported that patient was doing much better, slept through the night with no issues. Patient was still sleep when father left for work that morning, however, father looked in on him. Per fathher, his color appeared good and he was sleeping well. Father wondered what gauge needle was used for the procedure. He stated he thought is was long and perhaps a 1 ½ ? size. RN assured it was a 1? needle as the type of set-up used was a complete set-up with a standard 1? needle.

patient was sitting on bench in front of pharmacy and fell out of the seat onto the floor. Patient became very dizzy, flushed, and vomited. This happened about 10 minutes after vaccine given and lasted for about 10 more minutes. After throwing up, patient drank plenty of water and rested in which they started feeling good enough to walk away with their parent.

Caller called on behalf of his grandson (12-year-old) who received the first dose of the Pfizer Biontech COVID-19 vaccine and on the same day he got the shot, he experienced headache, neck ache, fever, and throwing up on 07Jul2021. Stated that the headache gets better and comes better. Stated that it was tolerable and comes back. Stated that the fever comes and goes. Started throwing up 2-3 days ago. She wanted to know if those side effects were related to the COVID19 vaccine and if she needed to go to the doctor and if he could get the second dose. Caller also stated that the side effects outweigh the COVID shot, I do not want my grandchild to suffer like that. Vaccines were supposed to make you feel better, not sick. Caller asked if the vaccine was tested on children and if they had side effects. Wanted to know if she needed to go the doctor because he cannot live throwing up all the time. Stated that he had never been sick before. Stated that he was only 12 years old. Does not think that vaccine was adequate for children. Therapeutic measures included Ibuprofen for his headache and fever. Stated that he had cold compresses for the neckache. Stated that he had ginger ale and Pepto Bismol for throwing up. Outcome of the event headache and throwing up was reported as not recovered at this time of the time. Outcome of the event neck ache was reported as recovered on 11Jul2021. A 12-year-old male patient received (PFIZER-BIONTECH COVID-19 VACCINE, formulation: solution for injection, Batch/Lot number: EV00191) via an unspecified route of administration in left arm on 22May2021 at 11:00 (age at vaccination 12-year-old) as dose number unknown, single for Covid-19 immunization. The patient's medical history was none. The patient's concomitant medications were none. The patient previously took Flu vaccine, 2 years before, onset Age was about 10 years and experienced flu symptoms. Patient was healthy, no allergies and no medical history. Not even regular vitamins. Patient did not receive any other vaccines on same date of the pfizer suspect. Patient did not receive any other vaccines within 4 weeks prior to covid vaccine. Patient's mother was a bit concerned. He was doing great and then on 23May2021, Sunday onwards he had chills. Then he had a temperature of 100 and she gave him plain acetaminophen and Sunday onward 6 or 7 hours onward he kept getting chills. On 23May 2021, for six to seven hours, he had chills. There have been temperatures of 90 and she has to give him a blanket or heater. It was more or less the same today 26May2021 Attempted to clarify that the caller stated the intensity was a little less. Caller confirmed. She thought it was a normal reaction and waited patiently for 3 days. She was not allowing the temperature to go up as soon as he gets chilled and breaks into fever, she checked his temperature and it was always less than 100 like 99.9 or 99.7 Fahrenheit, axillary. Then he broke into a sweat and feels a little better. He also had tiredness and loss of appetite on 24May2021. She also reported that on 22May2021, his arm was sore and difficult to lift and then it disappeared in one or two days. He took the vaccine on 22May2021 in the morning and then by Monday symptoms were gone, the soreness, and warmness at injection site. Patient experienced tiredness on Monday 24May2021 in the morning after fever, the lack of energy and tiredness started. On 24May2021 patient experienced loss of appetite. His breakfast, lunch or dinner reduced to one fourth. On 22May2021 in the morning started in the evening, around 6 or 7 o'clock, patient experienced soreness at the injection site. On 22May2021 in evening, patient experienced warmness at injection site. The time range for adverse events was 3 to 5 days. 3 days of fever no one gives her answers. Pediatrician says wait and after 4 or 5 days they look into it. Adverse events did not result into emergency room visit and physician office visit. Treatment received for chills was gave acetaminophen every 6 to 7 hours. Treatment received for fever was Tylenol every 6 to 7 hours. Treatment received for tiredness and lack of energy was keeping him hydrated and his appetite was quite less and she was giving him lemon juice keeping fluids. Treatment received for event warmness and soreness at injection site was gave him ice and it helped a lot localized. Patient did not receive any treatment for event loss of appetite. The patient underwent lab tests and procedures which included fever was 99.9 and 99.7 Fahrenheit on 23May2021. The outcome of event chills was unknown. the outcome of event pyrexia, tiredness, loss of appetite and lack of energy was not recovered. the outcome of event soreness and warmness at the injection site and difficult to lift his arm was recovered on 24May2021. No follow-up attempts are possible. No further information is expected.

I administered the Pfizer COVID vaccine to the patient at about 2pm. I completed the vaccine, put the used syringe into the sharps container, grabbed the bandage, and turned back toward the patient. As I did this, he started slumping over in the chair, and his eyes rolled back into his head. I immediately put my arms around him so he wouldn't fall out of the chair. The patient's father was in the room as well and knelt down in from of him. The patient's body spasmed for a few seconds, then he came to but was breathing heavily and starting to sweat. I called to technician in the pharmacy and asked him to call 911 at 2:03pm. I brought the patient a cool cloth and asked a technician to bring some water. The patient's breathing started to slow, but he was nauseous and dizzy. He dry-heaved once, and I put the garbage can in front of him. He slumped over the garbage can for a few minutes. Then he asked to lie on the floor. He seemed to do better lying on the floor. When the EMTs arrived, they attempted to have him sit in the chair again, but his dizziness returned, and he complained of being cold. The EMTs moved the patient to the gurney and transported him to the Hospital for further medical attention.

Patient received vaccine and began to feel nauseated and lightheaded. Patient began throwing up. EMT responded. Vitals BP 118/98 O2 sat 98%. Patient and mom refused EMT accompany to hospital. Will wait in observation for 30 mins.

Several minutes after receiving vaccine patient vomited. Monitored patient and he was fine after 15 minutes

PATIENT'S PARENT ADVISED TO PHARMACIST PATIENT HAD FAINTING/PASSING OUT SYMPTOMS IN PAST WITH AN HPV VACCINE. PHARMACIST HAD PATIENT REMAIN SITTING AFTER VACCINATION. WITHIN A COUPLE MINS, PATIENT'S MOM FLAGGED DOWN PHARMACIST, REPORTING EXTREME SWEATING, NAUSEA, TROUBLE SEEING. PHARMACIST CALLED 911 AND EMS ARRIVED WITHIN 10 MINUTES TO MONITOR VITALS. PATIENT WAS RELEASED FROM EMS CARE AND ABLE TO WALK/LEAVE PHARMACY WITHOUT AN ISSUE.

13 year old male who received his first dose of vaccination stated that he felt his throat was swelling and nausea. Vital sign were taken and they were all within normal limits including Oxygen saturation of 100%. No tearing, nasal discharge, no rash, No wheezing per EMS. EMS evaluated the sound of heart and the lungs and did EKG on the site. He was alert and oriented in place and time. He was able to communicate. EMS were on site and no medication were given. Patient and his parent chose to go to the ED by ambulance. He was stable when he left with the ambulance.

Immediate reaction in left arm site of injection deep red rash, skin blotching non-itching . Neck and face rash. Deep purple rash in both feet and toes. Given 25 mg Benadryl po with resolution of rash in 3 hrs. C/O headache and nausea.

pt had just received vaccine and found by ems crew with father at check out station. pt was seated with head in hands with complaints of light headedness and dizziness. pt was pale. pt was brough to evaluation area. pt suffered brief vomiting event. initial vitals were bp 85/50 hr 69 spo2 100% ra rr 18. pt was believed to ave been suffering from vaso vagal event.pt apperaed to recover quickly. pt second vital set were 90/60 hr 69 spo2 100%RA RR 18. pt skin color returned to a pink color. pt began to "feel much better" pts father gave consent to all evaluation and treatment. pt was believed to be stable. pt care was released. pt father was informed to contact emergency services if any symptoms returned and/or worsened.

Patient had convulsive seizure, became unresponsive and was cyanotic, sternal rub, patient opened eyes. Heart rate was thready. Patient felt nauseous. Patient was disoriented for 1min and quickly regained orientation. Patient was given water, patient laid on the floor. Discharged to ER Past Medical Hx Hx of seizure at age 4

Roughly 7-10 mins after the patient received the vaccine he complains of black out and cant see anything for a couple minutes and feeling weak and nausea. I measured the patient blood pressure and it seems lower than normal. After the patient rest for a while I reassess the patient blood pressure and he seemed to feel a lot better. I asked the family to stay for additional 15 mins or 20 mins to observe him but they insist on leaving . I instructed the parents to observe him and if he gets worst get medical help. I have a staff member at the facility to call the family after 30 mins later and the parents stated he is moving around and feel a lot better. The parents also called back a few hours later and stated he is doing good.

after administering vaccine patient started to vomit and drop in his pulse, dizzy and bit shakier. after observation called 911 and paramedic came and after checking everything falls within normal limits and patient went home.

Fatigue, pain around injection side on the day after vaccine during the day. Nothing concerning but right before he went to bed he didn't feel good. My son thought he had to vomit but couldn't. He said his HEARTBEAT WAS SO MUCH FASTER than normal. I got so concerned that I slept next to him which I never did before. He fell asleep after 1 h and woke up the next morning and felt fine. I did not go to the doctor because he was feeling well the next day.

His mother was present and told us he had multiple allergies to foods and sulfites, as well as asthma. He used to carry an epi pen but does not currently, also has a history of asthma for which he carries a rescue inhaler. He received the Pfizer vaccine at approximately 9:15am in his left deltoid, and about 2 minutes later Mom notified me and the school nurse that he was starting to get "hives". I noted a localized, small area of erythema on his left upper arm around the bandaid, approximately silver dollar sized, not raised. School RN removed the bandaid. He did not complain of itching and at that time stated he felt like he was breathing fine. The school RN gave him 50mg of Benadryl PO per the mom's request at 0920. The patient was in no distress, breathing and speaking normally. At 0922 he stated he felt like his throat and tongue were swelling, so I had him remove his mask, and asked mom if his lips looked swollen to which she responded "yes". He stated it felt like it was hard to swallow. We placed a pulse ox on the patient and his oxygen saturations were 96-100%, HR 85-90, RR 30. We administered an adult epi pen (0.3mg of epinephrine) into his left lateral thigh at 0923 and had the school office call EMS. The patient remained in no acute distress, and said the swelling in his tongue felt better after the epi, but that his throat still felt swollen. Lungs clear bilaterally, RR remained 24-30, HR 110-115, pulse ox 98-100%, erythema on left upper arm resolved. Fire department arrived at 0935- report given and they checked a pulse ox as well. Patient walked to parking lot to ambulance for transfer to Hospital with mother following behind.

Pt c/o lightheadedness, dizziness, flushing/sweating, nausea. Pt vomited 200mL digested food. Pt given PO fluids. BP 93/42, HR 66, RR 20 -- > 105/57, 59, 18 -- > 108/60, 65, 16 -- > 109/68, 68, 16. Pt was COVID positive Jan 2021 (s/sx body ach). No h/o fainting with vaccines or blood draws. Pt's symptoms resolved after drinking water and laying down with knees bent. Advised taking 2nd dose supine.

c/o difficulty shallowing, no resp. distress noted. O2 Saturation=98%, Pulse regular and steady. Able to drink water and masticate a chewable Benadryl 25 mg. Mom opted to leave and monitor pt. after 20 minutes in observation.

Patient became nauseous 15 minutes after receiving vaccine,then vomited,then fainted. We lowered him to floor with legs raised. He regained consciousness after a few seconds,but was still not clear-headed. He was very anxious and shaking and pale. 911 was called . He was taken to ER.,but was clear headed,with heartrate slightly elevated.

Vomiting; This is a spontaneous report from a contactable consumer (patient). A 13-year-old male patient received BNT162B2 (PFIZER-BIONTECH COVID-19 VACCINE; lot number: EW0187 and expiration date was not reported), via an unspecified route of administration in left arm, on 17May2021 at 16:45 (at the age of 13 years old) as 1st dose, single for COVID-19 immunization in a pharmacy/drug store. The patient's medical history was not reported. There were no concomitant medications. The patient did not receive any other vaccines within 4 weeks prior to the COVID vaccine. Prior to vaccination, the patient was not diagnosed with COVID-19 and since the vaccination, the patient was never tested for COVID-19. The patient experienced vomiting on 19May2021. This event resulted in doctor or other healthcare professional office/clinic visit. The event was non-serious. It was reported that the patient received an unspecified treatment for this event. The outcome of the event was recovering at the time of report.

Pt brought to obs room after getting vaccinated. Pt vomited, flushing, sweating. BP 103/61, HR 67, RR 14. 12min later, pt reports feeling better w/o symptoms. Able to drink 16oz of water. Pt reports fear of needles. Educated pt to let staff know for 2nd dose to be taken to obs room for vaccination. Pt and family verbalize understanding. Pt stable and released from vaccination site w/ family.

Vertigo starting 4 days after the shot with no other symptoms up to then. The vertigo exhibited as nausea when moving head that resulted in vomiting. Any movement of the head results in dizziness and nausea. No other symptoms in the rest of the body, no fever, no headache.

He started vomiting just before midnight and he was vomiting violently for about 6 hours. The next morning, I kept him home. Vomiting was slowing down. There was no fevers as of yet. He was very tired. He started having abdominal pain. We made an appt with the doctor. The next day, he started having fevers to 101.9. I gave him Motrin and that brought the fever down. He started food and drinks again. The day after all of his symptoms resolved.

Pt became pale, diaphoretic, and nauseous. Vomited undigested food once and complained of dizziness. Pt was laid down on the floor with his feet up on a chair. His color immediately came back, and he felt better and went home.

Patient was given the shot at 9:36 am and was asked to sit down for 15 minutes to watch how he reacts with the vaccine. He had been very nervous and anxious before getting the shot because he never had a shot before and was afraid of needle. After around 5 minutes, his dad notified me that his son had upset stomach. I asked the patient if he ate or drank anything in the morning. His dad and he confirmed that he didn't eat or drink anything (because dad thought that he should not eat anything before getting the shot like getting blood drawn). He confirmed that he breathed okay and didn't have swelling of lip and throat. His father told me that his son did feel a little bit dizzy and sweating. Since the clinic is full of patients and humidy, I told him and his dad to sit in a room in clinic without patients and bring a fan to the room. After 15 minutes (around 9:51am), after drinking water and eating a banana, all the side effects were gone. He felt better, and he could watch videos/games and have full focus. At 10:24 am (which was more than 30 min resting in clinic), his father and he confirmed that he was feeling better and didn't have any side effects and decided to leave the clinic and go home.

A 13-year-old male patient received first dose of bnt162b2 (PFIZER-BIONTECH COVID-19 VACCINE, Formulation: Solution for injection), via an unspecified route of administered in left arm on 21May2021 13:30 as single dose for COVID-19 immunisation at School or Student Health Clinic. Medical history was reported as none. The patient did not receive any other vaccines within 4 weeks prior to the COVID vaccine. Prior to vaccination, the patient did not diagnose with COVID-19. Since the vaccination, patient had not been tested for COVID-19. The patient did not receive other medications within 2 weeks of vaccination. The patient's concomitant medications were not reported. Patient had no allergies. On 21May2021 22:30, the patient experienced Sudden chest pain radiating toward armpit, dizzy, sweaty, rapid pulse rate, nausea. The symptoms resolved after about 10-15 minutes. This repeated a second time on Saturday 22May around 4 pm. Repeated a third time on Sunday 23May around 3 pm but was less in severity. Lot number on card not clear but appears to be EN0185 or EW0185. The events assessed as non-serious. No treatment was received for the events. The outcome of the events was reported as unknown. A 13-year-old male patient received BNT162B2 (PFIZER-BIONTECH COVID-19 VACCINE, formulation: solution for injection, Lot number: EW0168, Expiration date: unknown) via an intramuscular route of administration in left arm on 17May2021 (age at vaccination 13-year-old) as first dose, 0.3 ml single for covid-19 immunisation. The patient medical history included ongoing depression and ongoing attention deficit hyperactivity disorder (ADHD). The patient's concomitant medications included escitalopram oxalate (LEXAPRO) 15mg daily, methylphenidate hydrochloride (CONCERTA) 36mg daily, escitalopram 15mg daily and vitamins nos. Consumer reported that she has been reading about reports that young men have been having myocarditis after getting the Pfizer covid vaccine and her son, who received his first dose on 17May2021 began having symptoms like heartburn and pain in his upper chest on 23May2021, and now the pain was below his neck and he was hurting all day before the date of event reported and on the day of report . Patient also threw up. On unspecified date patient was in bed. Patient said primarily now this was like in his upper chest and throat and he was pointing to upper chest now. consumer was watching a news with her husband on day of report (tonight), the story on a news about some heart information and young vaccine recipient and patient was 13 years old and consumer was trying to work out whether there was something the consumer need to go emergency room over. Consumer just did not know if this related to patient's never ever complained chest pain, so consumer was just worried about. Consumer was asking if patient's symptoms were due to heart inflammation. She asked if this was due to heart inflammation and asked if she should take him to the emergency room. Consumer reported that she just spoke with DSU to file a report, no reference number for that encounter. Reporter stated that she was working on prescription and there was NDC. Treatment included consumer just gave patient some pills same and omeprazole 20 milligram and set it on other to. She gave on the night before event reported (tonight) and on the day of report. Client received the 2nd COVID vaccine of Pfizer (Lot# EW0182, expiration 06/28/2021). At 1729, Client reports nausea and wants to vomit. RN, RN, RN, RN, and RN respond to this event. The registered nurses placed Client in an anti-gravity chair and provided an emesis bag. RN responded to this event at 1730. Client appears pale, diaphoretic, states being nausea, wants to vomit, and dizzy. At 1730, vital signs are heart rate 89, oxygen saturation 99%. At 1733, vital signs are blood pressure 110/82, heart rate 62, oxygen saturation 99%. Client is offered supplemental nutrition and fluids. Client accepted both. At 1735, heart rate is 61 and oxygen saturation is 99%. At 1735, PHN responds and speaks to mother to educate her about ED precautions and common adverse effects. Client's mother reports Client has a history of ADHD, takes medications as needed, no known allergies, had a long day, as not ate much today, and was nervous prior to vaccination. At 1736, vital signs are blood pressure 110/78, heart rate 66, oxygen saturation 99%. Client reports no chest pain, no blurred vision, no headache, no shortness of breath, nausea is improving, and skin tone is returning to appropriate coloration of ethnicity. At 1742, vital signs are blood pressure 117/82, heart rate 82, oxygen saturation 99%. Client reports no new symptoms and improvement of nauseousness. At 1753 vital signs are blood pressure 112/68, heart rate 86, oxygen saturation 99%. Client reports all symptoms has resolved and is now hungry. Client stood up with no complaints at 1754. At 1754, Client walked out of facility with a steady gait accompanied by his mother.

8 days after administration, patient suffered extreme stomach pain. Upon presentation to the ER, it was determined he had appendicitis. An emergency appendectomy was performed.

Woke up with headache at 9am. Fatigue, achy, sweats and chills starting around 10am and ongoing at present time of 245pm. Vomiting three times (as of now) between 12pm and 245pm.

Immediately following the vaccine, my son felt totally fine. The pharmacist told us to walk around the store for 15 minutes. 8 minutes after the vaccine, my son said his arm felt tingly. He immediately began sweating profusely and said that he felt dizzy. I made him sit down and he then said that he felt nauseous. Within 5 minutes of the onset of symptoms, he vomited. He was pale and his forehead was cold/clammy. The pharmacist said it was anxiety and nothing to be concerned about. However, my son said that he did not feel worried, anxious, etc. prior to or immediately following the vaccine.

Fever - up to 102 - lasted until yesterday - it was back down to normal yesterday. He has had a sore arm - at injection site; the arm also was hard - a lump that was a little bit palpable - which is gone now; - body aches - gone now, chills, difficulty swallowing. He didn't have trouble breathing, but he felt his throat tighten when he swallowed (still has this); upper right abdominal pain - that is the real bad symptom still. His eyes hurt when he moved them around. Fatigue. He's been glued to the couch since Friday. I had given Advil and Tylenol. After the prolonged fever, I eventually called into the doctor's office. The pediatrician said it was common and said to wait it out but seek treatment if it got worse. He is able to eat but is awful nauseous. He is eating what sounds good. The pain stays the same when he eats.

Rash on chest and back still present, gave antihistamine allergy medicine. Headache, stomach ache, body ache, injection site soreness, still ongoing

Auditory and visual hallucinations, confusion, headache. Symptoms waxed and waned for approximately 15 hours and were preceded by chills, mild fever, myalgias, malaise and anorexia.

Pt started with mild cold sxs the day after the vaccine. Then 2 days after the vaccine in the evening developed chest pain - mid sternal - when going to bad. Vomited x1 and pain resolved and fell asleep. Next day also with an episode of chest pain in the pm but more severe and also with n/v; took to ER but left before evaluation done bc pain resolved. The following day 2 more episodes of chest pain less severe. 5 days later sought care for follow up- EKG and labs abnormal so sent to ER for further evaluation.

Next day, started with nausea in the M (8:00), then fever (12:00 PM), then chest pain (3:00 PM). Went to the ER to be monitored. On Sunday, said pain moved from injection site to under arm (small lump).

About 5 minutes after administration pt felt lightheaded, dizzy, and nausea. Gave pt water and smelling salts.

While giving a Pfizer Covid vaccine today, a patient appeared to be fine but after a few minutes suddenly passed out and had a short 3-4 second seizure on the ground. He did hit his head and appeared to be hypoxic. We called 911 immediately and informed store management. The paramedics asked him to walk off on his own power, but they did offer to escort the family to Hospital which I believe was accepted. His mother and sister were present too.

3 days after the vaccine, the patient woke up at 530 in the morning with sharp chest pain that lasted about 1 hour. He was also short of breath and the pain was worse when lying down. The day after the vaccine, he had fever and emesis, but that resolved within 24 hours. He was taken to hospital where he was noted to have a troponin of 6.611, and CRP of 1.8. He was transferred to another Hospital for further care. He was treated with IV Toradol. He was seen by cardiology and infectious disease. He showed improvement with nothing more than NSAIDs, and since his MRI was normal, he was not given IVIG.

Patient presents to the ED with myocarditis s/p dose #2 of Pfizer COVID-19 vaccine. From note: Patient is 13 yo M who was in his usual state of health prior to getting his Pzifer COVID-19 vaccine #2 on Saturday morning (6/5). Saturday night he began to feel fatigued with fevers as high as 102.8 F. These symptoms continued through the night and into Sunday morning when he had episodes of NBNB emesis. Sunday night he was actually feeling somewhat better and was able to sleep through the night. He stayed home from school on Monday due to fatigue. At 3 am this morning (6/8), he woke up with aching parasternal non-radiating chest pain that he describes as an 8/10 and worse while lying down. Denies SOB and heart palpitations. His parents took him to a hospital where he was found to have an elevated troponin (reported 5.84 - unknown scale) and Hct 28.8. There was also question of an abnormal EKG (?p wave inversion). He got a bedside echo which showed no pericardial effusion. Additional labs included: WBC nml, plt nml, MBCK 16. In the ED, his vital signs have been stable. EKG was normal sinus. Trop elevated to 0.43 and echo was normal. Reassuringly his echo was normal, he has had no arrhythmias (after nearly 24hrs of continuous monitoring), his chest pain has largely resolved and his troponin has remained stable.

Morning after vaccine the patient developed fever, myalgias and then developed abdominal pain and vomiting. Presented to emergency department where he was noted to have elevated inflammatory markers, gallbladder thickening, and decreased EF in the 40s. Patient met criteria for MIS-C and was treated with IVIG and steroids. Case was reported to a federal agency.

Vomiting, can't eat or drink without vomiting 1 day after shot. When tried to eat, vomited repeatedly. Other symptoms include fever, achy limbs, and a severe headache and sleepiness.

A 13-year-old male patient received the first dose of BNT162B2 (PFIZER COVID-19 VACCINE, lot number: EW0164, expiration date: 31Aug2021), at the age of 13 years old, in left arm on 14May2021 16:00 at single dose for covid-19 immunisation. The patient medical history was not reported. The patient was a normal 13 years old, he doesn't take anything, he's active. The patient did not receive any other vaccines within 4 weeks prior to vaccination. The vaccines administered on same date with BNT162B2 was none. The previous immunization with the Pfizer vaccine was none. The patient previously received the flu shot every year and experienced no adverse event. On 16May2021, about 48 hours after the vaccination, the patient started having stomach pain in the middle of the night. It began 16May2021 in the evening and into 17May2021, the patient was having stomach pain and not feeling well. Reporter stated the patient woke her up. Reporter stated they thought it was some food he ate. Reporter stated they tested his covid antibodies and they also personally at home on 17May2021 night, they tested him with a home kit for covid just to double check because nobody really brought it up and it was negative also. Reporter confirmed the home covid test was done on 17May2021. Reporter stated there was no known exposure, they just did it to make sure he didn't have covid. The patient was taken to the emergency room on 18May2021. Reporter stated they were waiting because patient felt good during the day and they thought nothing was wrong, but then the extreme stomach pain started again and so on 18May2021, she took him to the local emergency room. Reporter stated they didn't do much other than push on his stomach, she was worried about appendicitis. Reporter stated they didn't worry too much, but she thought maybe it was his appendix. The doctor examined him and said he had a stomach issue and released him with medicine and that was the 18May2021. On 19May2021 and 20May2021, the patient would get intermittent stomach pain, again thinking it was a stomach bug or something. On 20May2021, the patient experienced petechial rash on his legs. Reporter called the pediatrician and again the patient was having intermittent stomach pain and kept having it intermittently. The patient would feel fine then feel sick again then on the 22May2021, the patient had more bad stomach pain and some vomiting so he wound up back in the emergency department, it was a different one than the first on 22May2021. The patient did a lot of blood work, his labs came back good, but he did not do a CAT scan, but reporter tried to do everything else, tested patient for covid, antibodies and everything and they sent him home again on the 22May2021. The patient had intermittent pain again throughout 23May2021 then on 24May2021, he went to visit the pediatrician just for a visit and they were thinking its Henoch-Schonlein purpura (HSP) which is an autoimmune response and they still didn't have a diagnosis for his stomach. Reporter stated patient was perfectly fine at the pediatrician, then when they got home, he started having lower left abdominal pain and he doubled over in pain and started vomiting. The patient vomited about three times in total, it wasn't like excessive vomiting. Reporter stated the pain would bring it on, he does have a rash, which is still getting a little bit worse. Reporter stated they took him back to the emergency room, they did a CAT scan and that showed he had inflammation in his intestines and his stomach so they kept him in the hospital. The patient was admitted on 24May2021 when he visited the emergency room for the third time. Reporter stated because patient was in pain, they admitted him to watch him. Reporter stated they were worried about the patient having intussusception with the bowel, where his bowel can go inside each other and can create a blockage, so they wanted to admit him to make sure that didn't happen. Reporter stated the worst of the stomach pain was on 25May2021, which would have been at 19:00. The patient had not had an episode since then, he hadn't had an attack like that since. The whole week before from 17May2021, or the 16May2021 when it started through 25May2021, it was very intermittent of a lot of pain. So far, knock on wood, the patient hadn't had it since 25May2021 at 19:00 and hopefully tonight it won't happen. Reporter stated it will be about 48 hours of not having the bad pain. Reporter stated he still has some pain, but not like where she had to rush him to the emergency room. The patient's vomiting ended on 24May2021 or 25May2021. He had recovered completely from vomiting so far. The patient was released on 26May2021 with a diagnosis of HSP, which was an auto immune response, and he'd never had this happen before. Reporter stated if she googled HSP, it said it can still get worse, but it was not the worst case of HSP. Reporter stated the patient has never had anything like this before, he was perfectly fine, they made it through covid and didn't get covid and he is a healthy kid so she thought Pfizer should know. The outcome of the events HSP, stomach pain, petechial rash was not recovered. The outcome of vomiting was recovered on 24May2021 or 25May2021. The outcome of the events inflammation in intestines and stomach, didn't feel well/feel sick, intermittent pain and lower left abdominal pain was unknown. No follow-up attempts are needed. No further information is expected.

Patient's mother called explaining that within 24 hours of receiving the vaccine, the patient had bright/flashing light in both eyes, numbness in his fingers and toes, chest pain, and stomachache. These events did resolve by June 9th when she called me however he still had a stomachache. I told her that these events would not be expected and that she needed to follow up with his primary care doctor. He received his first shot from us as well with no complaints

13 yo M with a history of autoimmune hepatitis who presents with 1 day of fever, headache, altered mental status, and vomiting. He received his 2nd Pfizer Covid vaccine on 6/12 and began having symptoms the next night. He began taking ibuprofen for headache and fever. His symptoms worsened through the day until he had vomiting. He was saying strange things and behaving oddly. He became very hot and agitated at home, prompting mom to bring him into an outside hospital ED. There, rectal temperature was 106F and he was noted to be combative. A CMP was obtained, which was mostly normal. CT of his head was obtained, during which he had an episode of left upper extremity shaking concerning for seizure. He was reportedly alert throughout. Patient is a 13 yo M with a history of autoimmune hepatitis who presents with 1 day of fever, headache, altered mental status, vomiting, found to have cerebral edema and elevated CRP. There is no acidosis, elevated lactate, or metabolic derangement, and utox is neg. The etiology could be related to the vaccine. Ingestions are also possible given the high fever and altered mental status - particularly serotonin syndrome. Infectious etiologies are also possible given the fever. Treatments: hypertonic saline given x1 EEG started Tox screen negative Ceftriaxone and vanc given

Patient received the vaccine 6/7 during the day, that evening into the next night he developed subjective fevers and chills. Overnight, he had several episodes of NBNB emesis and had some chest pain. By 6/9, his fevers, chills, and chest pain resolved. However on 6/10, he began having chest pain again; this pain was in the middle of his chest, without radiation. It was worse with laying flat and walking around. He went to his pediatrician, who obtained an EKG and checked some labs. Patient had an elevated troponin of 2.04, CKMB of 12, ESR of 31, and BNP of 39. His EKG was normal but he was directed t come to the emergency room for further workup. Repeat troponin that evening showed up trending troponin of 4.77 and patient was admitted for further workup and monitoring. His chest pain resolved and his troponin levels downtrended through the next day (4.77 -> 2.3 -> 0.4). Patient was monitored on telemetry and had no events of arrhythmia. Echocardiogram obtain did not show any evidence of decreased cardiac function. Patient was scheduled to under cardiac MRI (at this time, results of MRI are pending)

Pt passed out and was very nauseous. He had just finished playing 18 holes of golf walking in extreme heat (90+ degrees F). Before passing out his face did go white and just looked like he wanted to vomit. He felt fine after laying down and drinking water and getting some sugar in him. Whole event lasted about 15 minutes but pt was passed out for less than 10 seconds.

A 13-years-old male patient received BNT162B2 (PFIZER-BIONTECH COVID-19 VACCINE, Solution for injection, Batch/Lot Number: EW0167) intramuscular, administered in Arm Left on 15May2021 17:00 (at the age of 13-years-old) as 1st dose, single dose for covid-19 immunisation at store. The patient had no other medical history and no known allergies. The patient's concomitant medications were not reported. The patient did not receive any other vaccines within 4 weeks prior to the COVID vaccine. No any other medications the patient received within 2 weeks of vaccination. Prior to vaccination, the patient was not diagnosed with COVID-19. Since the vaccination, the patient has not been tested for COVID-19. On an unspecified date of May2021, the patient experienced pain in arm, headache, nausea, fever at 100.2F. The patient did not received treatment for the adverse event. The outcome of the events was unknown.

A contactable female consumer (patient's mother) reported that a 13-year-old male patient received BNT162B2 (PFIZER-BIONTECH COVID-19 VACCINE, Solution for injection, Lot number not available), via an unspecified route of administration on 16May2021 at 12:30 PM (at the age of 13 years old) as first dose, single dose for COVID-19 immunization. The patient's medical history and concomitant medications were not reported. The patient experienced pain at the injection site, vomiting and feeling tired on 16May2021. The reporter said that her son's second dose was scheduled on June 6, 2021. The outcome of events was unknown. Information on the lot/batch number has been requested.

Patient is a 13 yo male, otherwise healthy, who received his second covid vaccine on Saturday. He did well until Tuesday morning when he started to have emesis and "chest pressure". He was brought to the ER where work up included a troponin level that was elevated at 20.43. Due to concern for myocarditis, he was transferred and admitted to the hospital for further work up and management. -Echo results note that the cardiac function and coronaries are normal. Very trace pericardial effusion -EKG at Good Patient consistent with pericarditis -Repeat troponin 15 Discharged 6/17

3 days after receiving Pfizer mRNA COVID-19 vaccine the patient acutely developed new abdominal pain followed by chest pain. The chest pain worsened in severity to the point that he presented to an Emergency Department and was found to have an elevated troponin, resulting in hospitalizaiton.

After the vaccination, within 5 minute the patient felt faint and started to lightly vomit. The stayed seated and over the next 25 minutes, periodically vomited small amounts and eventually started to feel better and in better spirits.

13 yo previously healthy x ADD, takes Trileptal, Welbutrin, Concerta and Latuda, received second Pfizer Covid vaccine 2 days ago. This morning, began with atraumatic mid-sternal chest pain. Pain is constant, not affected by position - ie no change with sitting/supine/standing. + fever to 100.5 tonight. + emesis x 1. Also had jaw pain that resolved after motrin. No cough, rash, diarrhea. CHEST PAIN - MYOCARDITIS secondary to rare complication of Covid Vaccine vs MIS-C Transport PICU nurse - spoke with Peds ED physician - they recommend IV and labs to evaluate for possible MIS-C.

fever (101.2 at home) and repeated vomiting starting a few hours after 2nd covid vaccination; guardian attempted to give Tylenol at home but patient immediately vomited so brought to ED for evaluation and treatment.

Patient felt better after 15 minutes BP 118/70, pulse 64. Advised patients father to consult with PCP before second dose. Advised to drink more fluid and keep hydrated. patient left for home with father without any symptoms of being lightheaded or nauseated

13 year old male patient who after two minutes of receiving the second dose felt dizziness, he appears sweaty and vomited once He sits in turn to get evaluated.

Conditions subsided by the evening of 6/25. Minimal effects by bedtime, tired, some body aches. Extra strength Tylenol was used for treatment

Patient became light headed and nearly passed out. Patient also vomitted twice. Patient was weak and feeling warm and clammy for about 15 minutes.

Patient (13 years old) came with his mother at 10:36AM for second dose of the Pfizer vaccination the Covid-19 clinics that are celebrated Wednesdays and the patient is not allergic to medications only seafood and the first dose there was no adverse reaction. When he was asked to wait for 15 minutes assistant that was in the Clinic informed me Pharmacist on duty that the boy felt dizzy and was vomiting. I went to their location with alcohol to inhale. We allocated the head between the legs. At 10:47AM blood pressure reading was taken BP= 112/70 P=101. Mother indicated he only had a juice for breakfast. At 10:52AM he was given liquid BENARDYL 10ml as anti-hemetic to vomiting. In this course we closed the treatment. As the patient was feeling better. We took another reading at 11:07AM BP=113/90 P=94. When the patient felt better 55 minutes he left with his mother. I called the mother of the patient at 4:18PM to know how patient continued and she told me after they left the patient went out to eat and was good and was sleeping. Assistant helped me.

On the night of vaccination, the patient experienced side effects of chills, fever and at 4:00, had started vomiting like 20 times and had severe stomach ache like sharp pain. The reporter went to the pediatrician of her son but that did not help and the pediatrician told this symptom is not a COVID vaccine (as reported). The reporter stated that the patient never got sick before and that her son was feeling better at the time of this report. The outcome of the events was recovered. Therapeutics measures were taken for events chills and fever and included acetaminophen (TYLENOL) and ibuprofen (ADVIL). Information on lot/batch number was requested.

1009: appeared diaphoretic pale and faint, vomited. 1012 B/P 98/60 02 sat 97, assisted to floor, elevated lower extremities 1015 102/74, pulse 85, sitting up, states feeling better, father at side. 1030 117/73, pulse 85, sitting up in chair, no c/o AO x3 no n/v. 1036 left with father, declined further medical attention. Advised to f/u with pediatrician.

Systemic: Vomiting-Mild, Additional Details: Patient's mother reported him vomitting a small amount in the observation waiting room following vaccination. Confirmed just small amount of fluid/stomach acid on rug. Patient was showing visibile signs of nerves prior to administration. Mom reports Patient's father moved Patient out side and Patient calmed down with no further events. Followed up a few hours later via phone and mom reports, Patient didn't have any ongoing issues. She did not report any additional adverse events/concerns.

He woke up with chest pain in the upper right quadrant at midnight and then it intensified when he was breathing deep breaths. They woke him up when he was sleeping and did not go away. Then, it continued through the next day and we asked our doctor if we needed to keep an eye on him because of all the reporting and stuff about the heart problems. They did a triage consult visit to see if it could be an adverse event from the vaccine. The course of action was to take 2 ibuprofen every 4 hours and use ice and heat to see if the pain improved to rule out musculoskeletal situations because that was what they thought it was. We did that for 2 days and then the pain moved to his back upper right quadrant and we noticed a rash all over his abdomen and we were not sure where he got that. Based on the fact that the pain moved the doctor was more confident that it was not an adverse event.

Client received HPV & Pfizer vaccine during immunization clinic @ 0830. Client was in observation area when he started c/o nausea. He had one vomiting episode, per RN. He was given an ice pack, blue emesis bag, water, and fan for supportive measures. He was in the observation area w/ his grandmother & sister. At approximately 0900, he reported feeling better and walked out of the clinic with his grandmother. EMS was not called. Client had vasovagal reaction.

1707- Patient in observation area and vomited on floor. Emesis white. Mask removed. Patient's lips appeared pale. Communicare team called for assistance. BP 128/88, HR 111, O2 sat 97%. Patient denies headache or dizziness. Denies further nausea. Reports feeling 'anxious'. Patient offered juice. 1711- HR 104,O2 sat 99%, RR 18. denies nausea, continues to report anxiety, patient sipping juice. 1718- Lips and skin pink and dry, denies further nausea. BP 118/82 HR 100, O2 sat 98%. Patient reports feeling better. 1720- HR 100, O2 sat 99%. Denies dizziness. Communicare provider discussed with family that reaction likely from being nervous and that he would 'do fine with his next shot". 1728- HR 100, ambulated to private vehicle with parent and this provider. HR after ambulation 104, no dizziness, O2 sat 99%. ER precautions reviewed via interpreter. Father stated understanding.

History of Present Illness: 13y/o male with no significant past medical history or allergies reports nausea following receipt of first dose Pfizer vaccine today. He reports associated pre-syncopal sensation, palpitations and minimal difficulty with swallowing. He denies shortness of breath, chest pain, swelling/numbness of the lips and tongue, vomiting, injection site pain/redness or rash. Physical Exam: VS- HR ~110 VS #2: taken by paramedic reportedly within normal limits and normal sinus rhythm Inspection: no redness, swelling or pain at injection site; no rashes. No tongue/lip swelling. Heart: increased rate, regular rhythm; radial pulse strong with increased rate Lungs: clear to auscultation bilaterally without adventitious sounds Plan: Patient was also evaluated by paramedics. After ~30min, the patient reported cessation of symptoms and was feeling much better. Paramedic informed patient and mother that he can be transported by ambulance to hospital or decline services. The mother and patient elected to decline transportation via ambulance to hospital as he was feeling much better. Patient and family left the site after his symptoms resolved.

Systemic: Dizziness / Lightheadness-Medium, Systemic: Fainting / Unresponsive-Medium, Systemic: Nausea-Mild

Patient received his second dose of covid vaccine on 07/12/21. He seemed fine and left our immunization area. Approximately 5 minutes later, his brother came back to the pharmacy to inform us that the patient was sitting down near the restrooms and not feeling well. I, along with another pharmacist, went to assess him. He said he was lightheaded and was feeling nauseous. We sat with him for approximately 10 minutes and checked his blood pressure twice. His blood pressure was high the first time we took it, but it returned to normal after a few minutes when we took it the second time. We subsequently found out from his mother that he fell/fainted in the restroom due to his dizziness. He was unhurt from the fall. After approximately 10 minutes he said he was feeling fine and was able to continue shopping with his mother.

Patient began vomiting about 4 minutes after getting his second dose of the covid-19 vaccine. Patient was instructed to have a seat. Within about 5 minutes he said he was ok. He stayed around for about 15 more minutes and said he was ok to go home.

Following vaccination, patient experienced syncope episode and fell to floor. He regained consciousness very quickly but felt dizzy and nauseous for a little longer. Observed patient and he was feeling better prior to leaving pharmacy.

fainted a few seconds after injection. mother stayed with him. recovered after a few seconds was nausous. gave water. ems came. said his pressure was low . they took him away for precaution. was alert and coherent when he left.

Patient became very faint and nauseous 5-10 minutes after receiving the Pfizer COVID vaccine along with his brother. When supplied with a bucket, paper towels, and a cold compress pack, patient's brother began to vomit . Patient may have also passed out during this time period. The store manager was contacted and emergency medical services were also contacted due to severity of the reaction. the patient was taken to a local medical facility as of the writing of this report.

Rapid fast rate for about 7 hours starting at 1:00am the day after vaccine dose 7/23/21. . has been experiencing milder episodes of fast heart racing about 2-3 times a day on 7/23,7/24/,7/25, and once today 7/26 Shortness of breath (started at 1:00am, felt better around 10:00am) has experienced once more on 7/24 shortness of breath Chest pain left side started at 6:00am on 7/23/21 and lasted for one day Nausea (zofran was given at emergency to treat his nausea) Tiredness still until today 7/26/21

APPROX 8-10 hours after receiving 2nd dose patient started to become noticeably disengaged in social activity The following days a major loss of apatite and further withdrawal in even the most intimate and immediate family social functions Complaints of delusions. voices giving him" bad" thoughts and" bad" visions. voices come as either "monsters" or "a little boy" We have noticed when he seems to be battling with these thoughts his eyes tend to go absent and twitches his mouth as if wanting to speak but at the same time not.

Mom states she found her son around 9 PM in the bathroom vomiting. She stated she moved him to the couch and he continued to vomit. She stated he fell off the couch and he was seizing. She called 911, when they arrived he was no longer seizing. When her arrived at the hospital, it was documented he was postictal and would not respond to painful stimuli. The hospital kept him overnight and are discharging him home today. He has fully recovered.

Nausea. BP 127/81; HR 76; RR 16; O2 sat 99%. Pt provided 16 oz water & time to rest in observation room. Sx resolve. Pt stable & released

45 minutes after vaccine patient got dizzy and felt like he was about to faint. Felt nauseous and had to lie down. Then he felt very hot but no fever. Three hours later he felt better but said he would have crashed a car if he had been driving..

At 0950, client requested assistance from emergency medical technician (EMT). EMT?s responded and brought gravity-chair. Client reported feeling dizzy, nauseous, blurry vision, and hard of hearing. Client was assisted into gravity chair. EMT obtained vital signs at 0954: blood pressure 110/78, heart rate 67, oxygen saturation 97%, respiratory rate 18, alert & oriented x4, skin was warm and dry to touch. Registered Nurse (RN) was alerted and assessed client. Client?s father accompanied client and reported client had no allergies or chronic conditions. Client is not taking any medications. Client reported eating cereal for breakfast. RN provided vaccination education and was encouraged to follow-up with primary care provider to report the reaction and to complete the survey on VSAFE. Vital signs at 1002: blood pressure 108/78, heart rate 71, oxygen saturation 99%, respiratory rate 20. Client reported symptoms decreased to a ?3/10? from a ?10/10?. Vital signs at 1007: blood pressure 112/68, heart rate 80, oxygen saturation 99%, respiratory rate 16. Client reported symptoms mostly resolved. Vital signs at 1014: blood pressure 108/70, heart rate 81, oxygen saturation 97%, respiratory rate 18. Vital signs at 1024: blood pressure 102/66, heart rate 65, oxygen saturation 99%, respiratory rate 18. At 1030, client stood up from gravity chair unassisted and walked out of vaccine site unassisted with steady gait.

Systemic: Fainting / Unresponsive-Severe, Systemic: Nausea-Mild, Additional Details: Patient received injection and got up from chair. After standing for a few minutes he fainted and lost conciousness. His mom and I got him to the floor and he regained conciousness within a minute. He said he felt thirsty and nauseated. His mom gave him water. He laid on the floor for 20 minutes and then sat up for a few minutes. Then he walked out of the dressing room and left the store.

Patient is a 14 YO male with no previous medical history that received the Pfizer vaccine administered into his left Deltoid at 1406. He was sent to the observation area where the nurse noted him looking pale at 1410. His mother was present during this whole event. He was taken over to the medical tent for privacy and further evaluation. He was complaining of being light headed. He was given apple juice and crackers as he had stated that he had not eaten or drank anything all day. At 1430 his VS read BP 132/80, HR 145, O2 99%. At this time he was lying down and feet elevated, shaking, pale, and cold. Lung sounds clear. Nausea but not needing to throw up he was covered up to try and keep him warm. At 1440 his blood pressure was 130/82, HR 130, 02 98% At this time he was lying flat with feet elevated, shaking, pale, and cold. His lung sounds clear. Nausea, but not needing to throw up. At 1450 BP 138/74, HR 120-135, 02 98%. Symptoms had not changed at this point except he mentioned his ears felt cold, although they were warm to touch. He was laid on his left side at this point to try and lower his HR, which did come down to 115-125 when on his L side. We did not take a BP at this point as we wanted him to be on his back when taking them. At 1515 he was laid back on his back to see how his HR would do and how his BP was. BP 138/60, HR 135, 02 98%. Again symptoms were the same except he was complaining of colors being more ?dull?, and his teeth being numb. At this point his pupils were noted to be +8, equal and reactive. 1521 VS read 138/50, HR 135, 02 98%. Vision still dull and pupils unchanged from 1515. He was laid on his R side to see if this would lower his HR. It was brought down to 120-130 on his R side. We gave him a cookie to snack on to see if that would help with any of the paleness, shakiness, and light headiness. He was able to take a few small bites. Hands pale and washed, with +1 bilateral radial pulses. 1529 he was sat up slowly to try to get him to hydrate, as it was apparent he needed fluids onboard. His BP read 142/80, HR 124, 02 98%. He was still complaining of lightheadedness. After sitting up and drinking half a bottle of water and some cookie, he stated that colors were becoming more vibrant. He noticed my eyes were very blue, which previously he had stated they were grey. He stated his lightheadedness had improved and that he was feeling better. We had him sit up for a while and his color started to come back, although his L arm was still pale, of note this was the Vaccinated arm. At 1546 his VS read 150/82, HR 117, O2 99% while sitting. His only complaint was feeling like the air he was breathing ?was cold?. At 1607 he was stood up and when doing so his HR went to 140. After standing for about 4 minutes his BP read 120/82, HR 114, O2 98%. His pupils +6 , equal, round, and reactive. He was feeling way better and his color in his L hand was back equal to his right. He was slowly walked around a chair just to see how he would do with activity. His HR was anywhere from 94-120 when walking around the chair, he complained of no dizziness, or other symptoms. At this point he had finished 2 apple juices, crackers, a cookie, and a bottle of water for a total of 1.5 L of fluid. He was sat back down in the chair to rest and see how things went for a while. His HR was anywhere from 109-120 when in the chair, although he did appear very anxious through this whole process. When he was given a task it appeared that his HR was a few points lower. His final HR was 109, BP 130/82, O2 98%. Of note he did complain of his feet and fingers feeling tingly a few times during this process. Sometimes his feet would be tingly when elevated and then when the garbage can holding them up was readjusted it resolved. These were intermittent complaints. At this point it is 1630. He has responded to fluids and is stable to go home with his mom. It was recommended for him to increase his fluids tonight, and to eat something. He was sent with a bottle of water. It was also recommended to get a pulse ox machine from a pharmacy so that his HR could be monitored and if it does not come down under 100 by tomorrow to make an appointment with his MD to follow up. If any of these symptoms were to come back or if his mom had any concern at all it was recommended to take him to the ED for observation. It was also recommended to follow up with his primary even though he left stable. Throughout this process, I considered administering Epinephrine. The problem was his BP was not low and we could not get his HR to come down. I did not want to increase his HR any more than it already was unless I was absolutely necessary. Given that he responded to fluids, it was decided not to administer it. Given that it took him 2.5 hours to recover from this vaccine, I strongly recommend that he does not get his second dose. I consider this to be an adverse reaction, and his mom was in agreement.

Nauseated after receiving vaccine, patient asked to sit in the wheel chair and accompanied by nurse to observation area. Patient feels better. Patient was observed for 30mins

In the first few minutes, I had a very mild sharp, needle-like pain. The next morning, my arm hurt very badly. Throughout the day, I had moderate fatigue. Also, I had nausea, dizziness, chills, and a feverish feeling (all symptoms were wild). That night, I ate a light, protein-filled meal, 325 mg of Tylenol, laid down for a while, and went to sleep. When I woke up, I feel perfect and had no symptoms.

About 5 min after receiving shot, his head went to the side, lost consciousness. His legs, arms, hands went stiff and he started shaking. His eyes rolled back in his head, textbook seizure. Lasted a little less than a minute. When came to he said he couldn?t see anything. After about 30 seconds he could start to see. He turned pale, felt like he was going to throw up. But never did. A few minutes later he started to get a little color back in his face. He is tired but was better.

Nausea and tightness in chest next day. Tightness is felt when relaxing and worsens when breathing deeper.

Pt reported muffled hearing, nausea, dizziness BP 117/59; HR 86; RR 20. Pt reported resolved sx after rehydration. Pt advised to get rest & increase fluid intake, advised to f/u w/PCP if other delayed s/sx. Pt stable & d/c'd from vaccination site

Client complained of palpitations 20mins after receiving vaccine. First vitals at 1637: pulse 147, blood pressure 124/78, oxygen 99%. Alert and oriented x4. Client denied chest pain, shortness of breath, headache, or blurry vision. Per Father client had asthma that resolved years ago, no current medications or allergies. Pulse at 1638: 127. Vitals at 1640: blood pressure 118/70, pulse 124, oxygen 100%. Per client palpitations improving. Alert and oriented. 1644: client hands shaking, client stated only had breakfast. RN brought client juice and snack. Client began to eat. Father denied client having diabetes. 1648 vitals: blood pressure 122/80, pulse fluctuating from 110-low 120s, oxygen 97%. Symptoms the same with new onset o blurry vision, per client could not focus image. PHN offered Father to call paramedics for further assessment and he declined. PHN advised for client to follow up with provider. Per Father client's sugar lowers when he skips meals. PHN asked Father is he has followed up with provider or checks client's sugar at home, Father denied. Per Father he knows his son and "he needs to eat or sugar goes low". At 1656 client stated he wanted to vomit, emesis bag given. 1658 client pale, PHN asked client how to was feeling. Per client feeling like he "was going to pass out". At 1700 client stated he could not feel legs, RN assessed for sensory and motor. Per client felt like legs "were asleep". PHN assessed for level of consciousness, client unable to answer, client lethargic. 1701 client lost consciousness. PHN and RN lowered client to floor from chair. EMT called 911 at 1703. RN did sternal rub, client unresponsive, breathing normal, pupils reactive. 1704 client woke up with urge to vomit. PHN and RN turned client on side. EMT able to place emesis bag near. Client had 1 small vomit episode. Client's eyes open, blank stare, not answering questions. At 1707 client went unresponsive again. Paramedics arrived at 1709 and assumed care. Client transported to hospital at 1717

Patient vomited and fainted. We already had EMS here for his sibling. He was also fine after resting and calming down.

Time to onset: 6 hours. Patient developed full body hives, severe congestion, lip, tongue, throat, and eye swelling. Wheezing. Difficulty swallowing. Treated with Claritin 20 mg po, Benadryl 25 mg po, albuterol inhaler 2 puffs, prednisone 50 mg po, and Asmanex 2 puffs. Within 1 hour hives and lip swelling began to resolve. 1 hr after taking prednisone, tongue and eye swelling resolving. As of 740 am on 5/19/21, hives resolved. Only continuing sx was mild throat swelling sensation.

Five minutes after administration, patient's father approached pharmacist and stated pt wasn't feeling well. Upon approaching pt, patient threw up. Nurse was called and patient had low blood pressure. Patient was told to sit longer and then patient drank water and felt fine after 25-30 minutes.

Patient vomited approx a few minutes after vaccine was given. Patient slowly fell onto the ground from the standing position. No injuries occured.

very swollen lymph node in neck, above clavicle toward the neck, left side (side of vaccination). Getting bigger every day for 4 days now. Overall soreness of shoulder (despite not having much injection site soreness the days after the shot)

Immediately after administration, patient became pale diaphoretic, lost consciousness, had jerking body movements, maintained pulse and airway. Once regained consciousness, vomited 3 times. Pt was given pepcid with fathers consent. After about 10 minutes started to complain of tightness of chest, short of breath. With consent from father administered 25 mg of benadryl. Monitored for 30 minutes. Patient status returned to baseline at the end of the 30 minutes observation. encouraged to follow up with pc prior to next vaccination to weigh the risk and benefits. Afterwards, dad said patient had reactions like this in past with vaccination. Asked multiple times if wanted EMS called or transport to medical facility. Declined by father.

Pfizer vaccine given 5/20/2021 Per mom/ pt the shot was given at 11:02 around 11:07 he leaned forward and became unresponsive. He was slightly drooling from the mouth. Mom says that his body was tight and that he was shaking a little. His eyes were closed and did not appear to be rolling back in his head. This episode lasted for less than 1 minute. He did not remember what happened and thought he wet himself after; which he did not. About 1-2 min after that episode mom states that he appeared to " passed out" for less than a minute. The ambulance was called and he was taken to hospital. He was evaluated and appeared well at that time. No fever. EKG was done and per pt it was normal. No EEG was done. He was discharged without admit That evening he has multiple episodes of vomiting > 10 times. No diarrhea or fever. Seen today at medical office for follow up. Vaccine not given at our clinic

Pt reported having trouble swallowing 12 minutes after injection, escorted to med ops 1, no evidence of allergic reaction, no rash, no trouble breathing. Vitals monitored as follows: LYING 5:59 - HR 85, Resp 12, BP 135/81, Sp02 100 Standing 6:06 - HR 64, Resp 12, BP 142/82, Sp02 100

Sore throat, headache, body aches, fatigue, cough, dizziness, Fever and nausea. Still ongoing. Went to clinic to get COVID tested it was negative. Seen by a nurse practitioner, gave him meds for cough and nausea. Still hasn't resolved 7 days later.

Pt received first shot of Pfizer vaccine on 5/14 - 12:30 PM EST. On 05/17 around 7:45 AM, Pt passed out/fainted. He woke up few minutes prior to the event and this event started after he passed urine. As soon as he finished Urination, he felt nauseated, dizzy and then lost consciousness thereafter. He fell backwards as a result and hurt his back as well as back side of the head. During his unconsciousness his body was completely stiff and his eyes were rolled back. For few seconds then he became completely still (it felt he was not breathing). I gave him physical stimulus (slight slaps on the face) post which he started reviving. During this time we called 911. I gave him few sips of water forcibly and he started reviving thereafter. on 5/15 and 5/16 (post vaccination on 5/14) he was feeling tired and seemed bit dull. This was unusual as Pt is a competitive soccer player and highly active.

Pt received vaccine and felt light headed and nauseated. He vomited and felt warm. BP 108/68, hr 98. pulse ox 96-99% Placed cool rags at neck and head, and given water, within minutes pt felt better. Was accompanied by mother.

Patient (14 years old) fainted in the observation area receiving the first dose of the Pfizer vaccine. Patient fell from chair and had some scratches to right temporal region. Patient was accompanied by his mother. Patient was assisted to chair and appeared very pale and disoriented to time and place. EMS was called at 1138. Patient also stated his vision was blurred, reported nausea (emesis x1), and reported feeling very weak and ?really weird?. Patient also reported he was very sleepy and wanted to go to sleep. Patient was kept awake. Mother stated patient had history of ADHD and anxiety but no other past medical history. Paramedics arrived at 1145 and assessed patient. Blood glucose and VS were normal. Patient continued to report feeling unwell and wanted to go to the hospital but also wanted to ?stop for food?. Patient?s mother reported that patient was starting to sound like his normal self and that his color was returning. Patient?s mother agreed for the paramedics to transfer patient to ED for further evaluation. Patient left in ambulance at 1157.

Nausea,chills,headache,body aches. They went away after the 4th day . He started having symptoms the night he got the vaccine.

HE SHORTLY CAME AORUND IN ABOUT ANOTHER MINUTE. NO EPINEPHRINE WAS ADMINISTERED.

5/16 Vaccination By 8 PM he started to have a stomach ache. He had went to bed. Went to school, was fine. Diarrhea started at home. 5:00 PM projectile vomiting. Continued with diarrhea for 20 minutes, uncontrollable. No temperature. *Was fine the next day.

Pt with presyncope event. Pt complained of sudden onset of nausea and feeling ?weird.? Clinician immediately at chair side. Initial vitals 102/58, HR 49, RR 18, O2 sat 98%. Pt assisted with receptacle for emesis-none. EMS medic 153 on scene and present. 4:25p BP 102/64, HR 62. 428p transferred to avail cot to lay supine. Aox3 and verbal entire time. Ice pack to neck. Water to drink without difficulty. Orthostatic vitals assessed ,WNL. Parent present throughout event. Mom refused transport. 446p EMS called verbal report to hospital MD as part of release for transport refusal. Permission granted to refuse transport . Pt up, walking around. States he now feels fine at 452p

Presented to the ED 6 days after receiving Pfizer vaccine with chest pain; pressure and burning with nausea and vomiting.

Systemic: Chills-Mild, Systemic: Headache-Mild, Systemic: Nausea-Mild, Systemic: Vomiting-Mild, Additional Details: Pt felt nauseated and had to the urge to vomit within 2-3 minutes of vaccination but never ended up vomiting. He also felt a slight headache and his temperature dropped from 98.7 F to 95.3 F within 10 minutes of vaccination. However, his temperature returned to 97.6 F approximately 9 minutes later and was monitored for total of 30 minutes with his temperature being 98 F when leaving the pharmacy. He felt normal with no symptoms upon leaving.

pt mom yelled for help. other employee dialed 911. mom reporting that patient keeps fainting/feeling nausea. used smelling salt x 2. then had patient have small sips of orange juice. patient began feeling nausea again and threw up. pt started breathing heavy/flushed/wheezing administered epi-pen (1st dose incomplete- bent needle) 2nd dose successful in muscle of right thigh. patient reported feeling better. took bp 94/54 pulse 88. ems arrived and took over. patient left with ems

Client received his 1st Pfizer vaccination (Lot# EW0187, EXP 08/31/21) in his left arm at 19:15 from RN. Client?s parents and sister were with him. Client?s parents only spoke Spanish and an interpreter was present. After receiving his shot, client sat in a chair next to his sister while RN administered a shot to her. Five minutes later client stated he felt nauseous. RN immediately called out to Vaccine Runner RN and Check-in Nurse who were in the hallway to get an EMT. Check-in Nurse radioed for an EMT to come to station 3 while Vaccine Runner RN rushed to the EMT room to ensure an EMT got the call. Check-in Nurse stayed with the client and placed a trash can next to him in case he vomited. RN stated the client looked pale and clammy. At 19:26 EMT1 arrived at station 3 and collected the client?s medical history. Interpreter was also translating the EMT?s questions and statements to the client?s parents. Client stated he had no known allergies, no underlying conditions, and is not currently taking any medications. Client also stated he was not experiencing shortness of breath, chest pain, or having trouble breathing. EMT2 arrived at station 3 at 19:27 and brought the client a vomit bag. The client had not vomited, but still felt nauseous. EMT1 gave the client a bottle of water and juice, and took client?s vitals at 19:28 (HR: 64, BP: 120/70). Client stated he felt a lot better. While walking out of station 5 Lead RN saw individuals grouped together at station 3 with the EMT and arrived at station 3 at 19:28. Lead RN was updated on client?s status. Lead RN observed that the client was mildly pale and clammy and asked client?s mother if his current coloring was normal for the client. Interpreter translated to both parents and client?s mother stated that the client did look a little pale. Client stated he felt better and was just nervous about getting the shot. At 19:30 Lead RN asked client to try and stand up. Once client was standing, Lead RN asked client if he felt dizzy and client stated no. Client stated he felt just fine. Lead RN and EMT?s directed the client to the observation room to sit in a zero-gravity chair for additional monitoring of 30 minutes. EMTs walked the client and his family to the observation room and sat him in the zero-gravity chair. Client stated he felt much better and at 19:47 EMT1 took client?s vitals (HR: 66, BP: 100/70, SpO2: 98). EMT1 stated client?s color was returning. EMT1 took client?s vitals again at 20:04 (HR: 67, BP: 104/78, SpO2: 99). Lead RN arrived in observation room at 20:10 with Lead Ancillary to check on client who stood up and stated he felt good. Lead RN observed that client was no longer pale or clammy. Lead RN educated parents and client on potential side effects vs adverse effects of the vaccine. Lead RN further instructed client and parents to see the client?s HCP or visit urgent care if client started to experience additional common side effects for more than 24-48 hours and to call 911 if the client began to experience trouble breathing, shortness of breath, or chest pain. Lead Ancillary translated all the information to the client in Spanish. Lead RN observed the client leaving with his family at 20:15. Client left walking with a steady gate.

Fever (high 101.7), chills, sweating, fatigue, headache, stomachache, nausea, leg weakness. Started upon waking in the morning 06/05, resolved by the time he woke on 06/06. Took Tylenol for fever twice on 06/05.

Flu-like symptoms consisting of myalgias and malaise started on 6/4/21 at 1300. Overnight into 6/6 around 0230, he began having chest pain that felt like severe heart burn and shortness of breath. Tried to sleep through it but was unable and sought care. Found to have elevated troponin level and transferred to our facility pediatric ICU for higher level of care. Currently hospitalized in Pediatric ICU.

A 14-year-old male patient received first dose of bnt162b2 (PFIZER-BIONTECH COVID-19 VACCINE, solution for injection, Lot Number: EW0179), via an unspecified route of administration, administered in left arm on 12May2021 at 17:00 (at the age of 14-year-old) at single dose for COVID-19 immunization. Medical history and concomitant medications were reported as no. Prior to vaccination, patient was not diagnosed with COVID-19. No known allergy. Patient did not receive any other vaccines within 4 weeks prior to the COVID vaccine. It was reported that 24 hours after the vaccine on 13May2021 at 17:00, while seate, suddenly both hands started shaking followed by heart rate increasing, reaching higher than 166 bpm. Breathing started to get rapid to a point of hyperventilation on 13May2021 at 18:00. Hands and stomach muscles cramped up as the heart continued to race on 13May2021 at 18:00. Called 911 and they were able to decrease the hyperventilation through slowed box breathing. They stayed for about 30 minutes as the heat rate slowly decreased to just over 100 bmp on 13May2021. Two other incidents of increased heat rate have happened since in May2021. Events were resulted in Doctor or other healthcare professional office/clinic visit, Emergency room/department or urgent care. Since the vaccination, patient had not been tested for COVID-19. Therapeutic measures were taken as a result of all events included hydroxyzine prescribed to induce calming effect as. The outcome of events heart rate increasing, hyperventilation and breathing started to get rapid was recovering; outcome of the other events was unknown. This report was reported as non-serious by reporter, with seriousness criteria-Results in death: No; Life threatening: No; Caused/prolonged hospitalization: No; Disabling/Incapacitating: No; Congenital anomaly/birth defect: No. No follow-up attempts are needed. No further information is expected.

2 days post vaccination patient developed hives--parent treated with Benadryl, 3 days post vaccination--patient's finger began swelling, worsening symptoms over following 4 days included increased difficulty walking (started with a limp), diarrhea, extreme pain with inability to walk Patient went to ER--admitted and given IV Toradol

Rapid onset of warmth feeling followed quickly by "visual changes" and dizziness. Brief syncope sitting without falling occurred with rapid recovery. Little to no diaphoresis or nausea. Low normal range BP's recorded in the range of 90-100 sys and 50-60 diastolic. Pulse 70-80. 02 sat -98% No orthostasis. Typical vasovagal reaction. Discussed with father & patient.

In first two days following second dose of Pfizer vaccine, patient had "normal" side effects of fatigue, low grade fever, chills, etc. On evening of day 3 patient developed headache, nausea, and severe chest pain. Emergency Room visit showed normal EKG an X-Ray but bloodowork showed troponin levels of 18,300 at 10:30 pm and 19,905 overnight. Patient was diagnosed with myocarditis and admitted to pediatric ICU at Hospital.

Within 5-10 minutes following vaccination patient nearly lost consciousness and felt nauseous. Parent states he has a history of vaso-vagal reaction following blood draws but never following vaccination. Patient was alert and oriented x3. Patient brought to cot to lay down. Vital signs taken x3 and were stable. Patient drank water and felt improved. Patient sent home with parent.

-Large red blotch on neck immediately after injection; resolved within 20 minutes, no shortness of breath or other signs of allergic reaction - Extreme fatigue next day (slept 15 hours) - diarrhea (moderate, x4 day on Day 2 and Day 3 after injection ) - stomach/abdominal cramps - feeling unwell day of injection, and on Day 2 and Day 3 after injection - headache - feeling hot (but no fever) - severe arm pain (injection site) - no appetite Note: it is now the end of Day 3 after the vaccine, and my son feels well.

The morning after the second dose of the vaccine he woke up and felt hot and my head hurt. He had woken up feeling similar a few times during the night. When he got up he felt really queasy and threw up. He felt really warm and continued to throw up. With Tylenol the fever was 104 degrees and it continued for several hours even after two doses of Tylenol. I was told to give Benadryl and Ibuprofen. The fever returned to 102 degrees and four later the fever broke. It was almost 10 hours where he was throwing up and the fever fluctuated between 102-104. It is very unusual for him to have any fever. We continued the Benadryl and Ibuprofen the following day because he still had a low grade fever and some dizziness followed by diarrhea. The dizziness and Diarrhea occurred again the next day three after the vaccine. It has been five days and he feels better but his stomach is still a little sensitive. It is possible that he had Covid-19 last year.

Pt received Covid #1 at out of office vaccine clinic on 5/27/21. Seen at ER 5/30/21 for behavior change (decreased level of responsiveness, staring off into space, vomiting & nausea, hyperventilation, anxiety Please see enclosed ER note. Spoke with Mom on 6/1/21 is improved History of Present Illness HPI Narrative: 14-year-old boy with history of autism presents with behavior changes, nausea, vomiting. Patient and his mother provide the history. He was in his usual state of health last night, had a normal dinner. Did not answer his door this morning. Parents found him staring off into space with decreased level of responsiveness. He was able to drink fluids. Patient reported having a few episodes of vomiting and abdominal pain earlier. They were concerned that he could be having a reaction to his COVID-19 vaccine which was given 4 days ago. Mom gave 25 mg of Benadryl at home. They were driving to Hospital for evaluation when the patient started looking worse. He has been hyperventilating but also has now clenching of hands and reports that his feet feel funny. Mom reports that he is not at his behavioral baseline currently. No prior abdominal surgeries or additional issues. Not currently take any medications. Patient denies taking any Illicit medications/substances.

Coughing, fever, chills, sweating, lethargy, decreased appetite, SEVERE nose bleeds lasting 20+ minutes, increased blood pressure and heart rate. Induced an asthmatic flare which required increased breathing treatments, steroids, and antibiotics. **Maternal grandmother had severe reaction. VAERS NUMBER 1182545

about 12 days after first dose, intense vomiting and diarrhea for 68 hours, which they ceased with no further issues. During the 68 hour period, all food and liquid resulted in symptoms within 30 minutes of consumption. No fever detected.

Patient developed vomiting and fever s/p 2nd vaccine. On day 3 s/p 2nd covid19 vaccine, developed chest pain.

COVID vaccine (Pfizer) given 5/13/21 and 2nd vaccine given 6/3/21. Both given in L-upper arm. No urticaria after 1st vaccine. Initially after 2nd vaccine he developed symptoms of tired, fever Tm 101.1 F, upset stomach, headache, body aches, chills and sensation that arms and legs were "numb" about 12 h after vaccine. The "numb" sensation lasted 30 min and no longer present. Denies enlarged lymph nodes, extremity edema. No hx/o COVID in the past. Went to Urgent care on 6/7/21 due to full body hives, so given benadryl and topical steroid triamcinolone. It continued to worsen during the day so taken to ER later that day and given Pepcid, steroid IM and benadryl. Discharged home on PO prednisone 20mg x 5 days. No Epi was given. Given benadryl every 6 hours and by the 4 hour mark it would come back. Given zyrtec twice daily 10mg/dose on 6/8/21 evening, 6/9/21 BID. Overnight didn't wake him for benadryl (last dose 8pm). On 6/10/21 given steroid, pepcid and zyrtec and seen by PCP in clinic for ER follow-up. No hives present on exam on 6/10/21 (present the night before). Hives would fade and then come back and were migratory. Hives were pruritic. Also used calamine. Once his lips were swollen, but denies any tongue swelling, itchy throat, cough, wheeze. Denies N/V/D. Mother cooked with a new crock pot seasoning at Farmer's market used at dinner the night before the hives developed. Also had strawberry short cake for dessert the night before. The day he had hives he also had strawberries and complained of stomach pain after eating them. No hx/o strawberry allergies or food allergies in the past. No knew meds, detergents, lotions, soaps prior to hives. No recent illness.

1258 Called to lobby. Pt pale, diaphoretic, and having tremors of extremities. Per mom he does this with vaccines. She did not say anything thinking it was psychological and if she did not mention it he would be OK. C/O dizzy. Transferred by parents to wheelchair then to treatment room. Two person standby assist to treatment table and lying in supine position. Verbalizes had not eaten and feels better lying down. 1300 VS 117-67 HR 76, Resp 14, O2 sat 97% room air. 1314 Skin color improving. Skin dry. Assisted to sitting position. 1315 VS 116/71 HR 71 R 14 O2 sat 98% room air. Dad assisted with ambulating to lobby waiting area. Instructed to sit for 20 min and will reassess. If dizziness returns to not stand up and let parent know. Verbalizes understanding

June 8th- received vaccine. Fatigue, arm soreness. June 9th- headache, decreased appetite June 10th- pain of both upper arms, substernal chest pain. One episode of emesis. Was admitted to our hospital for presumed COVID-19 vaccine related myocarditis.

06/08/21 2nd dose of Pfzier.Vaccination administered @ 10:44. According to client arm fasciculation ? started one minute after vaccine. Nausea 2 minutes after. Complaints of dizziness. Respirations 24 at 10:59. Respirations 28 and irregular at 11:04. Tightness in throat @11:10. Tongue not swollen. Breath sounds unobstructed(report given via school nurse), RN, PHN 06/09/2021 Mom reports child had fever and lump in throat the evening of event. The next morning child vommited x1. RN, PHN 06/11/2021 Mom reports no new side effects since the morning of 06/09/21 and the child is feeling fine. RN, PHN

About 3-5 min after vaccination patient felt unwell, hot, dizzy, feeling of passing out, had his head down between hands. Pharmacist and patients mom got patient down on the ground flat with legs elevated on chair, a cold pack and water. Pharmacist did not witness loss of conscientious. Patient was nervous about getting vaccine. We called 911, emergency services came, did a work up. It appeared to be a vagal response, possibly from fear of needles. When emergency services got patient up he seemed a little nauseous. They took him to hospital.

On the 5th day patient woke with nausea and extreme exhaustion and weakness in legs. After a few hours nausea subsided but limb weakness persisted. Patient had a hard time walking unassisted. Visited Dr office in early afternoon where neurological assessment occurred. Patient limb strength was normal while seated. All other tests passed. Patient tested negative for covid in pcr exam that day. Symptoms increasingly improved over next few days and symptoms are non existent 1 week following second vaccination on June 5th

Immediately following vaccination on 6/11 the patient developed left arm pain which became bilateral and a headache. These symptoms resolved with Advil. On 6/13 he developed self-limited epigastric pain and later that evening he developed acute midline inferior sternal pain, which prompted ED evaluation.

On June 12,2021 patient woke up (10am) and felt chest pain, nausea and loss of appetite (poor oral ingestion. He was brought to Health Center after evaluation patient was transferred to Hospital. On June 12, he was admitted to the Pediatric Intesive Care Unit with elevated troponins levels. Patient continue hospitalized chest pain and other symptoms has been resolved. Inpatient medications: Toradol 30mg IV q6hrs PRN for severe pain (7-10) Ibuprofen 40mg PO q8hr PRN Famotidine 20mg Iv q12hr Zofran 6mg IV q8hr PRN for nausea

14 y/o gentleman with PMH only significant for R orchiopexy 3 years ago who presents for persistent right inguinal pain and lymphadenopathy. The patient was last seen on 6/10 for right inguinal pain with workup significant for right inguinal lymphadenopathy on US and normal scrotal ultrasound. Patient was reportedly started on antibiotics by his PCP with no improvement. He reports overall decrease in appetite and nausea, but no emesis. He has been febrile at home up to 101F.

a 14 year old male with no significant past medical history who was in his usual state of health until he after his 2nd dose of Pfizer COVID-19 vaccination on Friday afternoon 6/11. On Saturday 6/12 he had headache, fever in the AM and had poor oral intake during the day and in the afternoon had developed mid sternal chest pain that Described as thumping and wrapped around his chest under his arms. He felt that it was worse with inspiration and continued to worsen despite tylenol. He went to a medical center where his workup on Saturday evening 6/12 was normal (including EKG and blood work). He also had some dizziness at that time but no syncope. He was changed from as needed tylenol to as needed motrin and instructed to take famotidine. On Sunday his chest pain persisted but was stable and he continued on motrin. On Monday 6/14 he had chest pain that felt like pounding this AM, headache, body aches, and pain in his bilateral arms, mouth and throat. He had some nausea but no vomiting or diarrhea. He represented to a medical center where his troponin was 0.25 ng/ml (normal 0-0.03 ng/ml) pro-BNP 324 (normal 0-125 pg/ml); ESR 9; CK 359; CK-MB 20.3; WBC 3.9; Hgb 14.1; plts 181. And he was noted to have ST elevations on EKG. He received toradol at 2:30pm with some improvement in his pain. His pain level has been variable 5-10/10; currently reporting that his chest pain is resolved. He has had no shortness of breath, palpitations, sick contacts, URI symptoms including cough, congestion, runny nose; no travel.

6/12 vaccine at 3pm, 10pm started shivering 6/13: headache, sore throat, nausea and fever by the evening 6/14 symptoms continued with the addition of chest pain, brought to the ER. Troponin 10, transferred to hospital.

Around 3 am he woke me up saying he couldn?t feel his legs and had a stomach ache. He got up in the morning to use the bathroom and almost passed out. Headache stomach ache , slight nauseous feeling lasted until about 12/1 pm. Very tired slept all day until around 6 pm. Very tired into the next day. Around noonish the following day he seemed to be back to normal.

A 14-years-old male patient received bnt162b2 (BNT162B2), dose 2 via an unspecified route of administration on 07Jun2021 (Batch/Lot number was not reported) as 2ND DOSE, SINGLE DOSE for covid-19 immunisation. The patient medical history was not reported. The patient's concomitant medications were not reported. The patient previously received first dose of bnt162b2 (BNT162B2) on an unspecified date (3 weeks prior on Monday) for covid-19 immunisation. It was reported that his son who was 14 years old got the 2nd dose (Covid vaccine) of Pfizer yesterday around noon. So that was around something 17 hours ago, the reporter don't receive any information on how to handle any side effect. He apparently has headache and he has fever for 101.5 right now and he has stomach pain. But you said they were not open right now well that this was not very helpful at this point because its 5 eastern time right now. So where could get information on how to handle or what we should do with him to help him. If the reporter could go someplace for reference and couldnot contact at this point in time. Does not seem appropriate and especially giving that you know, you could do on a certain time. So pretty upset with that the reporter known it was recorded one. So, reporter hope someone on side does something about that because it was not very good practice he would try to find answers elsewhere. The outcome of the events were unknown. 14-year-old male presents to the emergency department 4 days after his second Covid vaccine. Initially after the vaccine he had some generalized fatigue and nausea but yesterday started experiencing some chest tightness lasting up to 4 minutes. He also feels an irregular heart rate or like his heart is bounding. He also had some generalized bilateral neck stiffness. This is all resolved currently. He did vomit 1 to 2 hours ago. No fevers or chills. No cough or cold symptoms. ECG shows normal sinus rhythm with ST 1 box upsloping elevation V3 through V6. Initial troponin is elevated at 2.6. Attempted to consult cardiologist and they recommended pediatric cardiology consultation. Spoke with Dr. . She recommended admission with trending troponins and Motrin. She says they typically do echocardiogram and if normal then cardiac MRI. Spoke with Dr. in the ER and they graciously accept patient in transfer.

Received 2nd COVID-19 vaccine on 6/14 and developed headache and myalgia that evening. On 6/15, continued to have these symptoms then developed abdominal pain and vomited once. Symptoms persisted on 6/16; then developed chest pain at 11:00PM and reported that pain seemed worse when inhaling and was sharp, shooting pain. Presented to ED at 12:00 AM on 6/17 and subsequently transferred to Pediatric ICU due to elevated troponin and d-dimer. Initial EKG showed tachycardia. Cardiologist consulted. He had an echocardiogram on 6/17 AM that showed mildly depressed LV systolic function with EF ~58%. Troponin was elevated from 2.9 to 16.9. Due to echo and elevation in troponin patient received 100 gm IVIG (max dose based on IBW). Has required one dose of 15 mg ketorolac IV and 650 mg acetaminophen PO x1 for chest pain. At 1:00 PM on 6/17, troponin increased to 54.6 and aspirin 81 mg started. Chest x-ray showed borderline cardiomegaly. On 6/18 repeat echocardiogram with normalized LVEF and considered a normal echocardiogram per cardiologist.

Systemic: Nausea-Medium, Systemic: Seizure-Severe, Additional Details: Patient received vaccine and within 2 minutes complained of nausea& lightheadedness to his mother then passed out. Upon putting him on the ground he began to seize. Paramedics called & came & took vitals. All within normal limits. Patient was alert & talking & stated he felt fine. Upon being sat up in a chair his eyes rolled back & he began to seize again, more violently. He was then placed on strecher & taking to ER.

fever to 101 F on 6/17/2021 at 8am which persisted. Chest pain started 6/18/2021 at 3am and has been constant. sharp chest pain lower chest midline, worse with laying, better with sitting or walking. Shortness of breath with laying also. Also has palpitations, light headedness, dizziness, nausea, feels clammy. Chest pain worse on 6/19/2021 at 1:30 am and came to the ED for further evaluation and treatment. Just got admitted and full evaluation in progress.

About 18 minutes after receiving the vaccine, the patient collapsed on the elevator and was unresponsive for a couple minutes. He reported feeling dizzy and nauseated, and he appeared pale. His blood pressure was 103/69, and his heart rate was 52. Of note, he had not eaten breakfast, nor had any water to drink on the morning of the event. He was given apple juice and some crackers, and his symptoms resolved within about 20 minutes.

14yo M initially presented to the ED with headache, fever, myalgia, abdominal pain, NBNB emesis, unsteady gait and soft/garbled speech, 6 days after returning from a recent trip to Mexico and 4 days after receiving 2nd dose of Covid-19 vaccine. In the ED patient had multiple tonic clonic seizures, total episode lasting about 5 minutes requiring ativan and keppra. Pt was admitted to PICU for management, found to have continued seizures on EEG resolved with fosphenytoin

Patient received the Pfizer vaccine (Pfizer Lot#:EW0217, 1st dose) on 6/23/21 at 1349. Patient alerted EMT and stated that he immediately started feeling nauseous at 1354. Patient vomited at 1358. Patient stated that he felt good post-vomiting. Patient was place on zero gravity chair and received water and snacks. EMT took patients vitals signs. BP: 108/68, pulse: 68, 02 sat: 98%. Patient stated that he got braces yesterday and was not able to eat much in the morning. Patient stated that ate a quesadilla at 1300. EMT retook patients vital signs and reassessed patient at 1409. BP: 111/76, pulse: 80, O2: 98%. Patient reported no new symptoms. EMT retook vital signs and reassessed patient at 1424. BP: 111/74, pulse: 62, O2: 97%. Patient reported no new symptoms. Patient had completed his 30 minute observation time at 1424. Before leaving patient was educated on worsening signs and symptoms of the covid vaccine and when to call EMS or go seek emergency services/urgent care. Patient left vaccination site with mom, unassisted and with a steady gait at 1426.

6/27/21 Developed left chest pain at 5 p.m. Took ibuprofen that did not really help. Slept fine. Chest pain returned at 11 a.m. on 6/28/21 while playing tennis. Chest pain increases with deep breath. He needs to take deep inhalation at times to feel comfortable breathing. No symptoms increased with laying flat. He had 1 Diarrhea episode after vaccine which he blames on food he ate. Next bowel movement was normal. No fever.

Patient c/o dry mouth, difficulty swallowing, chest felt heavy and fogginess. Spoke to Mother of pt she stated pt is fine, she gave him claritan and the symtoms resolved. I advised he shoud wait 30 minutes when recieving dose #2

My son experience after the vaccine around 03:40pm, he felt dizzy, started to black out and passed out. I gave him fluids and called his PCP. I made him eat since he did not eat earlier that day. He states he felt off, I gave him some Tylenol medication. He also had a headaches and started with diarrhea. The next day he stayed in bed, continued with flushed bright red cheeks, diarrhea and the headaches. The day after that he started to feel bit better, I think his symptoms lasted between 36-48 hours. His glucose level was 90.

He had vomiting, headache on 6/26/2021 and became comatose that night and got better after steroids. He is still weak in the right arm.

Pfizer COVID vaccine (6/23 dose 2). That evening he developed chills. . He developed body aches, decreased energy level, and decreased appetite. He woke up with extreme chest pain that radiated to the lower portion of his neck. He felt his heart racing and found it hard to take deep breaths secondary to pain. Brought to ED for further evaluation. He was afebrile, 99.4. Cardiologist on-call, was contacted and recommended transfer to another facility. In the hospital ED

Ten minutes after patient received vaccine, patient parents informed the pharmacy that he was not feeling well. Patient was visibly sweating, holding his stomach and then started vomiting. Father then informed the pharmacy that the patient had experienced vomiting after a previous vaccine but had cleared it with pediatrician to get the COVID vaccine. After several minutes patient stated he was feeling better and left accompanied by parents.

Intense nausea and vomiting starting at 5 AM, intense achiness and headache. Vomiting intermittent throughout the day, with 20-30 occurrences between 5 AM and 10PM. Ondansetron 4mg RX administered at approx 9 AM and 8 PM. No improvement after morning dose, contents promptly thrown back up. Brief period of improvement late afternoon until early evening on 06/29/2021. Water and crackers and toast administered during improvement. Child was vomiting bile by 10PM. As of this report, not yet resolved.

Medical history was none. No Known allergies. Prior to vaccination, the patient was not diagnosed with COVID-19. There were no concomitant medications. The patient did not receive any other vaccines within 4 weeks prior to the COVID vaccine. The patient received first dose of bnt162b2 on 13May2021 16:30 (lot number: ew0186) at 14 years old in left arm for covid-19 immunisation. The patient experienced severe chills, headache, nausea, vomiting for approximately 1-1/2 hours on 09Jun2021 23:00. Treatment received for the adverse event was aleve pm. Since the vaccination, the patient has not been tested for COVID-19. The outcome was recovered. This case was reported as not serious. No follow-up attempts are possible. No further information is expected.

myocarditis with troponin elevation and arrhythmias diagnosed 7/1/21 after episode of chest pain and nausea

PATIENT EXPERIENCED NAUSEA APX 5 MINUTES AFTER HIS VACCINATOIN. HE VOMITED. HE COMPLAINED OF COLD CHILLS AND A TINGLING IN HIS ARM. WE OBSERVED HIM FOR APX 30 MINUTES AND HE WENT HOME WITHOUT MEDICAL INTERVENTION.

Patient vomited after approximately 5 minutes he had received Pfizer 1st dose vaccine. Patient left by ambulance at the mother's request for further observation. Pt left in no distress. Patient went to Hospital by ambulance unit.

Pt mother called and stated that pt is vomiting and has a fever of 101.9 pt mother state that pt received the second dose of COVID vaccine on 6/29/21 and now he is having these symptoms. Pt mother state that pt had these symptoms before with the first dose however they resolved on there own. Pt is now complaining of chest pain is unable to take a deep breath because it causes him pain. Patient Has not attempted treatment at home. CHIEF COMPLAINT: VOMITING (one episode of vomiting this morning) and FEVER (fever started on Wednesday) Assessment/Plan DIAGNOSIS at time of disposition: 1. Acute myocarditis, unspecified myocarditis type 2. Acute chest pain 3. COVID-19 virus not detected Patient presents with chest pain 2 days after 2nd COVID-19 vaccine. Labs were obtained and his troponin is elevated to 122 and his CRP is also elevated. I discussed case with ID, recommended admitting to Cardiology for workup for myocarditis. 14 y/o previously healthy male who presented to hospital after onset of chest pain, fever, chills, and vomiting this morning. Found to have elevated troponin and mild ST changes on EKG. Patient recently received 2nd Pfizer-BioNTech COVID vaccine on 6/29. Admitted for close monitoring and further work up of suspected myopericarditis following COVID vaccination.

Patient felt as though he was going to pass out. Became diaphoretic, pale, nausea. Patient was given juice, cold pack applied to neck and forehead. Placed on his back. He felt better about 5 mins after treated.

Patient reports onset of chest pain three days after his second Pfizer COVID vaccine. In the following three days after his vaccine, he had headache, decreased appetite, and mild nausea. However was still taking in fluids, decreased solid food intake. He also experienced insomnia and generalized malaise. At midnight on 07/02 he began to experience 9/10 substernal chest pain that was sharp, burning in sensation and located in the middle of the chest, non-radiating, pleuritic which lasted for several hours. Patient has had no fevers, vomiting, diarrhea, congestion, sore throat or cough. Denies any SOB, dizziness, syncope, palpitations, edema, orthopnea. There has been no trauma to the chest. No recent URIs. He was seen at a hospital where he was found to have a trop of 1251 ng/L (converting to ~1.5 in our system) then one hour later up to 1500mg/L. His EKG at the hospital demonstrated lateral ST elevations. Had a normal CXR. He was given Maalox which was not helpful. He was transferred to another medical facility after 3 hours at the hospital. Upon presentation to the emergency department, his vitals were stable. He had elevated CRP to 7.69, troponin was 1.0. CBC and Chem were normal. BNP normal at 15. Bedside ultrasound showed no pericardial effusion. Cardiology was consulted and ECHO showed mild left ventricular dysfunction with EF 47%. Repeat EKG here showed ongoing ST elevations in lateral leads, though mild. His pain has since improved to a 2/10. He has not required any medication for his pain. He is COVID/flu/RSV negative. On arrival to the floor, his chest pain had completely resolved. Patient reports the pain was pleuritic, worse with exertion and improving on rest. PMH: idiopathic urticaria Allergies: NKDA Family history: none reported, no cardiac disease or sudden cardiac death Hospital Course: Patient was admitted to the cardiology floor for continued monitoring of his troponin levels and EKG. He was then treated with IVIG 2g/kg and started on methylprednisolone IV x 2 doses. He initially had some very mild chest discomfort that improved by day 2 of admission. An echocardiogram was performed which showed improvement in his EF to 54%. Given his dysfunction, he was started on Enalapril 2.5mg twice daily. He was transitioned over to PO steroids which he tolerated well. On day of discharge, his troponin levels were trending down, and EKG was WNL. Cardiac MRI was performed and showed late gadolinium enhancement in the left ventricle. He had occasional PVCs but no arrhythmias. Viral myocarditis studies were sent and all negative. By time of discharge, patient was well-appearing, vitals stable, demonstrating good PO intake. Prescriptions sent to preferred pharmacy. Follow up with cardiology in place. Discharge instructions and return precautions reviewed with patient and parent, who expressed good understanding and agreement with plan. Patient will follow up in cardiology clinic next week with a repeat MRI 3-6 months.

Pt. reports that a few hours after the vaccine, he felt that his throat and chest were tight and uncomfortable and then he developed fever, chills, myalgias after vaccination that resolved after about 48 hours. Last documented fever was Tuesday (7/6) morning. In general, he has been feeling tired with decreased appetite but maintained adequate hydration and has had normal urine output. Over the past 24 hours, he has developed chest discomfort that has progressed to chest pain; it peaked this morning (7/8) when he woke up and he woke his parents up because the pain was so bad. Due to chest pain, he presented to the ED for evaluation where he was found to have non-specific ST segment changes on his EKG, elevated troponin to 5.69. He was admitted to the hospital and treated with NSAIDs where he has been improving.

~10 mins post vaccine became diaphoretic, described a buzzing in his ears, and was not aware of where he was, talking throughout, no LOC, BP stable 100's/60's, sats 98-99%, no respiratory distress, vomited a moderate amount and immediately felt better. Seen by PCP on site immediately following episode.

Felt faint, hand numbness/tingling, turned pale in color, nausea 15 minutes after receiving 1st dose of Pfizer COVID vaccine. Patient put head between legs and a cold compress on neck. Took blood pressure 20 minutes, 30 minutes, and 35 minutes after vaccination. Blood pressure range 93-102/62-68. Pulse range 76-104.

Prior to administration of the vaccine, I discussed with mom and patient about previous immunizations and the YES response to previously fainting or felt dizzy post immunization. Mom proceeded to state that he has a strong vaso-vagal response to needles/immunizations and he has fainted multiple times during and after immunizations. The patient appeared calm, so I did some breathing exercises with him and when I determined that he was calm, I administered the vaccine. Patient was still sitting upright, I asked him to continue to breathe slowly and deeply. I asked if he would like a water, and so I went to grab it from the pharmacy lab. Patient then stated he was dizzy and was going to pass out. He did not pass out, and I asked him if he would like to lay down. I put down a towel for him and then he started to throw up. I got a cold washcloth for his neck. He started sweating profusely (essentially a marked cholinergic response). I got him to lay down after some more vomiting . He stated that his vision was blurry, but he was laying down calmly. after 25 minutes of reclining, he stated he was starting to feel a bit better. I offered a 25mg diphenhydramine tablet to help with residual nausea. He stated he was better and go shop with his family. They did not return to the pharmacy, so as of my last interaction, he was fully recovered from the event.

Left arm pain, vomiting, Sickle Cell Crisis. Was admitted 7/10 thru 7/12 to hospital for pain management and IV fluids and bloodwork.

Client reported "burning" in his arm around the vaccination site immediately after receiving the COVID vaccine. The client reported "numbness" around the deltoid where he received the COVID vaccine. No noted redness or swelling around the site. At 1:35PM the client reported a headache, stomachache, and left arm pain from his elbow to shoulder. The client described the arm pain similar to sore muscle pain. The client denied any s/s of anaphylaxis. The client stated "I feel the food in my stomach." Vitals obtained at 1:38 PM were as follows BP 141/84, HR 95, and O2 98%. The client stated he went to bed at 2:00AM last night and did drink water this morning. At 1:40PM the client requested, was provided, and began consuming a snack and juice. At 1:48PM the client said, "I feel good." Vitals obtained at 1:49PM were as follows: automatic BP 172/73, HR 95, and O2 98%. Noted the automatic BP cuff was placed incorrectly. Repeat automatic BP obtained at 1:532PM was 151/82. At 2:03 the client reported that his arm was still sore but his headache was improving. The client denied any dizziness, nausea, or chest pressure. Vitals obtained at 2:03PM were as follows: automatic BP 162/83, HR 90, O2 97%. At 2:07PM education was provided to client and his mother regarding elevated BP, s/s of anaphylaxis, when to seek EMS. The client and his mother voiced understanding of this. The client stated that he felt nervous. Recommended the client and his mother follow up with PCP regarding elevated BP. Client's mother voiced understanding of this education. Assessed upper extremity strength, which was equal bilaterally. Assessed facial nerve function which was equal bilaterally. The client denied any shortness of breath, dizziness, or itching. Examined the vaccination site, and noted no redness or swelling. Manual BP obtained at 2:09PM was 150/86. Vitals obtained at 2:24PM were as follows: manual BP: 150/78, HR 86, and O2 96%. Offered to call EMS to evaluate the client. The client's mother declined EMS at 2:29PM.

a few seconds after client reported to RN feeling dizzy at 1715. RN set up anti-gravity chair and client was able to transfer without assistance. RN then informed PHN client was not feeling well and prior to vaccine had stated he had a headache. Mother had told RN client's headache was minor and to still administer vaccine. PHN arrived at 1717, client alert and oriented x4, sitting in Fowler's position, pale. Vitals at 1717: pulse 95, oxygen sat 99%. Per client new onset of nausea and stomachache, RN gave client emesis bag and water. Blood pressure at 1719: 118/76, pulse 75. Per client's Mother, client has medical history of migraines, no current medications and no known allergies. Per client had ate earlier today, but spent all day out in the sun. Client stated his migraines are often accompanied with nausea and vomiting. Client informed he would be kept 30 mins in observation. Vitals at 1724: blood pressure 120/72, pulse 75, oxygen sat 99%. Per client dizziness same, headache slightly improving 5/10. At 1726 client reported needing to use restroom. EMT guided client to restroom. Client able to transfer self again back to anti-gravity chair. Vitals at 1735: blood pressure 125/76, pulse 78, oxygen 99%. Client reported feeling "slightly dizzy", headache improving and nausea had resolved. Client stayed with EMT remainder of observation. Vitals at 1741: blood pressure 120/78, pulse 70, oxygen sat 99%. Per client headache 3/10, dizziness and nausea resolved. At 1746 client reported mild headache. Vitals at 1810: blood pressure 120/78, pulse 72, oxygen sat 99%. Per client headache same. EMT educated client and Mother to follow up with medical provider and gave ER precautions. Client left facility with steady gait at 1812

Adverse Event: Guest had complaints of shakiness, Nausea, "Out of body experience". Treatment: Guest assisted to the floor, then transferred to medical area, provided two apple juices - reported he had not eaten and he experienced the above adverse event after he saw blood from his injection site. Outcomes: Guests reports feeling better after after 15 minutes and above interventions. Left facility with his mother via POV.

First dosage: sore arm Second dosage: Headaches, chills, body aches, nauseated, sore arm, and dizziness,

24 hours after the shot he had nausea, stomach pain, headache and a sore left arm. He felt completely fine by the next day, about 48 hours after the shot.

Patient was alert but disoriented, difficulty breathing, speaking in between breathes, chest tightness, dizziness, and nausea 5 minutes after receiving the Pfizer Covid 19 Vaccine. 911 was called & paramedics arrived at the scene about 10 minutes from onset of patients symptoms. Patient was taken into Paramedics care and transported to the hospital.

Systemic: Dizziness / Lightheadness-Medium, Systemic: Fainting / Unresponsive-Severe, Systemic: Vomiting-Mild, Additional Details: Patient had fading consciousness 5 minutes after vaccine administration. When patient became unresponsive to questions and care, Epipen was administered. Patient recovered quickly, ambulance was called. Patient went with ambulance.

Patient received his first dose of the Pfizer covid 19 vaccine and after a few minutes passed out briefly whiles waiting with his mom in the pharmacy lobby. He complained of nausea. He was able to walk out of the pharmacy and left home with no other issues after the paramedics checked him out.

Approximately 20 minutes after the vaccine the patient was the passenger in his mother's car when he started feeling confused, experienced blurred vision and reportedly lost consciousness. Upon regaining consciousness, the patient vomited.

Patient had throat swelling potentially difficulty swallowing. Patient was given Benadryl and patient improved. Reaction occurred 30 minutes after dose. Patient has history of allergies and Asthma and had similar reaction to peanut in past. Vital signs were all stable throughout the course. Patient will follow up with physician allergist this week

Pt had just received vaccine, was walking to waiting area and experienced a syncopal episode, head stuck floor. Pt appeared to posture, skin was cool, pale, and diaphoretic. C collar applied by ems, PT TF to stretcher from floor and TF to EMS room. Vitals obtained, BP 150/90, HR 76, RR 20, SpO2 98% RA, pupils PEARL. Pt complaint of head pain and nausea. Pt's father advised that Pt needs to be TP to hospital by Rescue and father agreed. PT care TF to Rescue crew.

Patient felt dizzy, hot, nauseous and subsequently fainted approximately five to ten minutes after vaccine administration. He regained consciousness within a few seconds. They had him lay on the floor with feet elevated. Dizziness and lightheadedness resolved. He remained under observation for 30 more minutes.

Pt had just received vaccine and stated he "wasnt feeling well", and laid down on the floor. Pt assisted from floor to wheel chair and vitals obtained : BP 150/80, HR 118, RR 20, SpO2 97% RA. PT complained of headache, nausea, and not being able to walk. Pt father advised EMS that pt "often get's emotional and works himself up like this" and did not show concern to Pt state. Pt stated he wanted to go to the hospital, Pt father insisted that he take Pt to hospital via POV. Pt father signed refusal of TP form, and PT care TF to Pt father.

Patient stated his throat was tightening up and that he was having difficulty breathing shortly after (~20 min) receiving the vaccine. Parent stated that the patient has a known allergy to peanuts but was assured that the covid19 vaccine from Pfizer does not contain any peanuts. I checked to make sure the patient didn't have any hives or swelling in any other areas of body and face. Patient was still alert and able to speak but did display some labored breathing. I had an EpiPen ready to administer but first gave him 50 mg of diphenhydramine liquid which he showed difficulty ingesting and continued to monitor while EMS was on their way. When EMS arrived they checked the patient's O2 and it was at normal levels. EMS determined patient was okay and gave the parent the option of taking the patient home or she could have him taken to the ER in an ambulance. Parent decided to take patient home.

AFTER RETURNING TO WAITING AREA, PATIENT BEGAN TO FAINT. IMMEDIATELY REGAINED CONSCIOUSNESS AND STAYED IN AREA. PATIENT WAS VERY PALE AND HAD BRIEF NAUSEA. BOTH HAD SUBSIDED WITHIN ABOUT 5 MINUTS. PATIENT CONTINUED TO STAY IN THE AREA FOR 20-25 ADDITIONAL MINUTES AND SINCE HAS ONLY A SORE ARM. PATIENT WAS VERY NERVOUS AND ARM WAS VERY TENSE UPON ADMINISTRATION.

12:15pm pt. was seen sitting in wheelchair with EMT and dad at his side. EMT stated pt. c/o dizziness, nausea and blurred vision after taking Pfizer COVID-19 vaccine. At 12:13 vital signs taken by EMT staff. HR 74, BP 104/70, O2Sat 99%. 12:20pm pt. denies any pain or discomfort. 12:30pm pt. left vaccine site to go home via wheel chair. EMT and dat at his dice with no signs of acute distress.

Patient had a brief syncopal event about 1 minute after receiving the vaccine. About 20-25 minutes after the vaccine he had 2 episodes of vomiting. He felt normal within an hour of the vaccine.

Patient had vasovagal event at Vaccine clinic. LOC < 10 seconds, Reported feeling dizzy, nauseous, and lightheaded. Patient was laid down on cot, VSS. Laid for 10 min, sat with legs dangling for 10 minutes. Given juice for sugar boost and water. Patient's mother present. States he has had episodes like this in the past. Patient is a vegetarian. Denies HA, difficulty breathing, SOB, CP. 1:08pm VS: BP: 84/62 O2: 98% HR: 52 - laying PERRLA, HRR, no vomiting. 1:13pm VS: 90/59, O2:98%, HR:54 Discharged at 1:30pm in stable condition in care of mother. Return/ED precautions given. -

Patient complained about feeling lightheaded and nauseous. EMT called. Vitals BP 76/53 HR 63 O2 sat 99% and blood sugar 112. Per patient's mom, that is his normal vitals. Patient given water and laid down in ambulance. Patient refused EMT accompany to hospital and states he is feeling better.

Patient felt nauseous and lightheaded several minutes after receiving vaccine while sitting in chair in observation area. Was given ice pack, water, juice and snack. Taken to health suite via WC and placed on cot, supine with LEs elevated to rest for several minutes. VSS. Symptoms resolved after several minutes and patient able to go home with father.

Patient received first dose of Pfizer vaccine. Instantly became nauseous and Faint/pale. Assisted him to the floor elevated legs. No LOC. Pulse was 80, BP 138/80

Client felt clammy, almost faint. Tingle in legs and felt nauseous. Evaluated by EMS. BP 100/60 P 72 Felt better and went home with mom.

Systemic: Dizziness / Lightheadness-Severe, Systemic: Nausea-Severe, Additional Details: patient developed dizziness and nausea while waiting in observation area post vaccination. staff checked patient's blood pressure and heart rate which were both within normal limits. patient instructed to lie down with head resting. after 30 minutes, symptoms subsided and no emergency services necessary.

Sunday afternoon(5/16) got Pzifer COVID vaccine Dad says on Monday am early they woke him for school and he was asking where was he and why they were waking him up- kinda like in a dream like state and confused looking; they told him to just get up- he was able to get out of bed but seemed out of it -this lasted a few hours- he went back to sleep and when he woke back up he was not confused anymore but very tired; he also had some nausea and vomited; No fever; no cough; no body aches; L arm is a little sore- not as much today; each day a little better has had a little headache- but not much of one; Last night had better appetite so ate a lot of food then vomited again; has not really have an appetite before last night ; this am able to eat and kept food down today; Sleeping more but was restless sleep until last night as well Back in school for the past month; No school work this week- tried some math this am but trouble focusing so just stopped doing work

Patient became pale, nauseous, blurred vision and lowered blood pressure. Ambulance called and patient was ok and sent home with parents. Next morning at 1030 Am patient was called and was fine.

Approximately 8 days after first shot I started to feel strange feelings in chest after spending multiple hours outdoors. It was a pressure like feeling. Occasionally when I swallow it will feel uncomfortable, but not every time.

Patient reported feeling "nauseous" about 5 minutes after having received vaccine. Patient had an episode of syncope in the restroom. Assisted patient to sitting position was A&O x4, no signs of active bleeding or cuts. O2 saturation was 99% with HR of 83. Patient was able to swallow and answer basic commands.

Pt dad thinks pt has vasovagal syncope. Pt passed out immediately after getting vaccinated and vomited when regained consciousness. Pt turned white in the face and fell forward in chair when he fainted and after vomiting pt continued to go in and out of consciousness. Took about 10-15 minutes of placing feet on another chair and raising hands over head but pt color returned and he felt much better.

At approximately 6 PM on 05/23/2021, family was sitting down for dinner when all of a sudden she noticed that he was turning his head to the left and appeared to be falling out of his chair. Mom states that eyes were darting back and forth at this time. He was not responding to her shouting his name and his left arm was curled in towards his body with the rest of his body remaining stiff. This lasted approximately 1 minute, followed by a time of confusion and drowsiness. Approximately 1 minute later mom states that patient's eyes rolled back to his head and he had full body movement consistent with tonic-clonic seizures. Family were able to lay him onto the ground during this time. Mom denies any color change, apnea, or loss of bowel or bladder. States he did have one episode of emesis following the event, and 2 episodes of emesis in the ambulance on the way to ED. Patient has no previous history of seizures or seizure-like activity. No new ingestions. Has not been sick recently and denies any new sick contacts. There has been no head trauma. Per Mom, patient's dad has a history of "partial seizures" but never required any medication for them. At arrival to the ED he continued to be drowsy and not at his normal baseline. Continue to hold his left wrist up toward his body in a contracted manner and tended to have repetitive speech. Briefly, ER course significant for a CT head and evaluation by pediatric neurology. CT head was unremarkable for acute intracranial process. A CBC, CMP, and coags were also obtained due to patient's continued contracture positioning and the possibility of a stroke. CMP was unremarkable with a sodium of 139, potassium of 3.7, chloride of 107, bicarb 25, BUN of 12 and creatinine of 0.64. CBC was also unremarkable with WBC of 10.72, hemoglobin of 14.8, hematocrit of 44.4 and platelets of 370. There was 81.77% neutrophils after evaluation by neurology it was felt that the patient remained postictal and recommended that he be admitted on continuous EEG and possible MRI in the a.m.

AFTER VACCINE WAS GIVEN ABOUT 5-10 MINUTES LATER, PATIENT WAS EXPERIENCING DIFFICULTY SWALLOWING , NUMBNESS IN MOUTH. TOLD PATIENT TO LAY DOWN, GAVE EPIPEN 0.3MG AND CALLED 911. STAYED WITH PATIENT UNTIL EMT'S ARRIVED. AFTER PATIENT WAS HELPED BY EMT, HE WAS ABLE TO WALK OUT OF THE RX. SPOKE WITH PARENT AT 330PM ON 5/25/21 WHO STATED THAT THEY TOOK HIM TO URGENT CARE WHERE HE WAS STARTED ON ZYRTEC AND GIVEN AN RX FOR AN EPIPEN. PATIENT WILL FOLLOW UP WITH ALLERGIST, AND I RECOMMENDED IF HE IS TO GET A 2ND DOSE, THAT HE HAS IT IN AN MD OFFICE. LEFT A MESSAGE WITH PATIENT'S PEDIATRICIAN ABOUT WHAT HAPPENED.

Patient exhibited signs and symptoms of syncope. Felt dizzy, clammy skin, nauseous. Had him lie down for 15 minutes with legs elevated and used an ice pack on the neck to help improve blood flow. Patient recovered.

PATIENT EXPERIENCED BLURRY VISION, SWEATING, NAUSEA, AND HEADACHE. PATIENT WAS GIVEN WATER AND GUM. PATIENT WAS REQUESTED TO STAY FOR LONGER MONITORING. WAS DISMISSED ONCE THE BLURRY VISION, SWEATING, AND NAUSEA SUBSIDED.

Ready to leave. Home with mother.

On 5/23 two days after vaccine he was irritable, tired, fatigued, not sleeping well. He could not complete his track meet . He had abdominal pain, felt bloated, and he felt this nausea & discomfort. He thought he might be constipated, so he took MiraLAX and then he had diarrhea all day yesterday. on 5/26 he had a temperature of 99.2, he had acute chest pain retrosternal and feeling of compression on the chest, head fullness as if he is going to explode. He had nausea, insomnia. Ibuprofen was given to him by his mother and this helped a lot But he woke up on morning of 5/27 , but the chest pain returned with increased pressure, very nauseous, agitated, unable to be comfortable. No fever,, the pain was a sharp pressure. He had no syncope or chest shortness of breath He was seen at Hospital ER where a chest x-ray was normal. Echocardiogram was done, ,noted to have EKG changes. Troponin was elevated. He had a cardiac catheterization done emergently at hospital through the right radial artery and was noted to have normal coronaries. He also had a chest x-ray done which was negative for pulmonary lesions, and his abdominal pain he says was relieved after the MiraLAX and the diarrhea. His twin sister who received the vaccine 15 minutes after him is completely asymptomatic.

Six days (evening of 5/20) after the first dose of the COVID vaccine (Pfizer), Patient started to experience severe dizziness and nausea. Since that evening he has experienced the following: - Significant increase in heart rate when transitioning from laying down to sitting and/or standing. - Severe dizziness - Excessive fatigue - Headaches - Gastrointestinal issues (constant nausea. Nausea increases in severity when standing. Diarrhea.) - Muscle pain in his upper arms and upper legs The above symptoms have continued 24/7 since their onset.

My son is very lightheaded and dizzy. Can?t get out of bed for 3 days. Nauseas and feels week. This comes in waves. Went to ER 2x and they can?t find anything causing it

patient received vaccine on 5/19/21. Did not report what happened to pharmacist until 06/02/21. Patient states he had " heart burn" after vaccine. He states, " chest was hurting, felt really tired". I asked if he had palpitations, pt responded " yes" . patient states symptoms happened day after vaccination and lasted one day and was over after 24 hours. Mother gave him tums for heart and it helped. patient is feeling well and not reporting any symptoms. advised pt to contact MD for follow up and have md clear pt for second covid vaccine if any concerns.

Within 30 seconds of administration, patient felt dizzy and lightheaded. He never lost consciousness, but he did stumble and almost faint. Mother caught him - she says he has a history of dizziness after vaccines and is nervous about needles. He was very hot and sweaty, and he did vomit once. He was given water and an ice pack. Epinephrine was on-hand, but not needed. After resting for 15-20 minutes, he felt better and mother took him home.

15 year old male with a passed medical history significant for Marfan syndrome, aortic root dilation, and ADHS transferred to our facility for further evaluation and management of chest pain with elevated troponin. Father reports patient recieved the first dose of the COVID-19 Pfizer vaccine on 5/21/2021. On 5/22 patient developed a mild headache as per father that self resolved with rest. On 5/23 as per patient he was feeling back to baseline with some mild body aches. On the night of 5/23 patient felt some mild chest pain prior to going to bed that had self resolved. This morning patient refused to eat breakfast, and complained of stomach pain. Patient took Tylenol, however began to complain of severe chest pain and asked father to take him to the hospital. Patient was taken to his Doctor where his labs were significant for a WBC of 6.6, creatinine of 0.62, normal LFTs, troponin of 7.26, CPK of 312, D-dimer less than 0.19. EKG at Doctor was significant for normal sinus rhythm with an ST elevation, and chest x-ray was negative for cardiopulmonary disease. Patient had an episode of emesis at Doctor. A troponin was repeated prior to transferring to our facility and it had increased to 11.8. Father/patient denies chills, fever, diarrhea, sore throat, nasal congestion, and cough. ID consulted for further recommendations on management of myocarditis in the setting of recent Pfizer COVID-19 vaccination. Troponin repeated upon arrival to our facility, and has improved with no interventions. Interval History 5/27/21 No acute events overnight. Denies chest pain, SOB, cough, or palpitations. Cardiac MRI yesterday describing inflammation related to possible myocarditis, along with stable findings of MVP and aortic root dilation. Troponin this morning dowtrending to 0.28. No fever. He received a Mg Sulfate bolus yesterday due to low Mag on BMP

This information was reported to this writer. Immediately after receiving the second dose of the Pfizer vaccine, the patient reported a "weird sensation" in his chest. The patient became pale, and reported nausea and lightheadedness. According to the vaccinator it seemed as though the patient went into a syncopal episode. A nurse attempted to do a sternal rub to wake the patient up but was unsuccessful. The patient was then laid onto the ground and woke up; attempted to sit patient up and then went into another syncopal like episode. EMTs were notified and were on their way to attend to the patient. Patient's feet were elevated. No history of vaccine reaction. Patient had eaten and had fluids 2 hours prior to the vaccine. Patient was sat up again with assistance but immediately felt lightheaded and had another syncopal like episode. He was again placed in a supine position with feet elevated. Immediately reported improvement of symptoms. Patient had several sips of juice and was placed on a stretcher and taken to a back room. Patient had mom and sister at his side. As the patient remained in supine position, he denied nausea and lightheadedness; no chest pain, no dizziness. Patient was alert and oriented. Remained pale in color according to his father who just arrived. Per the advice of the EMTs, patient was transferred to hospital per ambulance.

Reports ate a few peanut butter pretzels today. Reported lightheadedness that resolved within about 1 minute, followed by seeing spots for about 2 minutes resolved, momentary tightness on inspiration that resolved, and ringing in ears for about 3 minutes resolved. Later reported nausea on ambulation post vaccination to seat in observation area that resolved upon sitting down. Also, reported has history of similar s/sx with flu vaccination. Denies chest pain and other s/sx. Nurses responded with emergency cart. 01:47pm LUE 96/70 pulse regular R20, lungs clear, lips noted to be dusky/purplish. Benadryl 50mg PO x1 administered. Lips color reassessed and after few minutes returned to usual pink color. Reports nausea, lightheadedness, visual spots, and ringing in ears resolved, all s/sx resolved, stated "I feel normal". Counseled follow up with pediatrician, urgent care or emergency department for any return s/sx, Mom verbalized understanding. Observed for 30 minutes. Ambulated with steady gait off floor with his parent and sibling.

it looked like the blood vessels or veins in the bend of elbow, inside of the elbow that looked like possibly a blood vessels that burst there.; feels sick; his body didn't feel right and he felt that way again yesterday when he felt so bad.; numbness and tingling right arm; tingling/numbness and tingling right arm; rash on the right arm; he has pain and a rash on his right arm; feeling like passing out; dizziness/lightheadedness; nausea; chest tightness/ chest was so tight and pulling.; pain in the arm at the injection site; headache; This is a spontaneous report from a contactable consumer (patient's mother). A 15-year-old male patient received the first dose of BNT162B2 (PFIZER-BIONTECH COVID-19 VACCINE), dose 1 via an unspecified route of administration, administered in Arm Left on 14May2021 11:15 (Batch/Lot Number: EW0173; Expiration Date: Aug2021) as 1ST DOSE, SINGLE for covid-19 immunisation (Age at vaccination: 15 years). Medical history included asthma, allergy, acne, pain, Gastroparesis, eczema, red blotches /bright red dots on his whole body and anaphylaxis. Concomitant medications included cetirizine (CETIRIZINE) taken for allergy; beclometasone dipropionate (QVAR) taken for asthma; mometasone furoate (MOMETASONE FUROATE) taken for allergy; isotretinoin (ZENATANE) taken for acne; paracetamol (TYLENOL) taken for pain; sodium chloride (AYR SALINE, both nasal spray and nasal gel) taken for allergy; herbal extract nos, homeopathics nos (SIMILASAN) taken for allergy, all are ongoing. The patient previously took advil [ibuprofen] and experienced anaphylactic reaction (when he was about 4 years old in about 2009 or 2010.). The reporter who is mother of patient calling regarding the Pfizer Covid 19 vaccine. States she wanted to call and report some side effects from the vaccine for her son. Reports he received the first dose of the Pfizer Covid 19 vaccine 14May2021 at 11 15 AM in the upper left arm. He stayed extra minutes after the shot because he had anaphylaxis before and had red blotches before in the past. Reports after receiving the first dose of the Pfizer Covid 19 vaccine, when they got to the car he said his chest was so tight and pulling. That lasted 1 to 2 minutes only on that day (on 14May2021, 12:00). Since then and even on the very first day he has had dizziness; lightheadedness; and felt like passing out (on 14May2021, 12:00). Adds he has been having to go to the nurse's office for headaches for the last two days (occurred on 14May2021). The reporter states her son has been very disappointed and mad at her for making him have the vaccine. Nausea - she adds her son also had some nausea (on 14May2021, 12:00). Right arm - he has pain and a rash on his right arm, the opposite arm from the shot, with numbness and tingling right arm (occurred on 17May2021); tingling. When he went to the nurse someone in that office mentioned that it looked like the blood vessels or veins in the bend of elbow, inside of the elbow that looked like possibly a blood vessels that burst there on an unspecified date. Pain in the arm at the injection site (occurred on 14May2021, 12:00) from the first day that is improved. The patient's body didn't feel right - when they started walking to the car his body didn't feel right and he felt that way again yesterday when he felt so bad. The first day and yesterday he felt his body didn't feel right (occurred on 17May2021, 11:45). Dizziness - is mostly when standing up and is lasting a lesser amount of time and getting better. Feels the sensation of like passing out with the dizziness on standing. Headaches - he has every day but yesterday was terribly bad and today were the worst. The reporter mentions that the patient stated the he feels sick on an unspecified date. Outcome of the events loss of consciousness, dizziness and vaccination site pain was recovering. Outcome of the event chest tightness was recovered on 14May2021 while outcome of the events numbness and tingling right arm was recovered on 18May2021. Outcome of the events pain in arm, vascular injury and malaise was unknown. Outcome of the remaining events was not recovered.

"1:26pm- 15y/o male reported he was feeling dizziness and was brought to the observation room. Pt vomited a small amount. 1:28pm - Pt was transferred to bed. VSS and WNL. BP 111/71, HR 83, RR 24. Pt had no prior hx of dizziness, vomiting after vaccination. Pt was checked by MD. No Resp distress, allergic rxn to vaccination was observed. Pt drank 16oz water and reported feeling better. 1:39pm - Second vitals 111/70, HR 84, RR 20. Pt walked around the room w/ PA and was communicating w/ father & RN. 1:41pm Pt stable and released from the vaccination site with father."

Severe headache 10 out of 10 started the following day with vertigo. During this time he developed an episode of sudden onset double vision and vomiting which resolved after approximately 10 minutes. The headache and the vertigo lasted for several days.

One day after vaccine (6/6) patient had fever, chills, vomiting and developed hemoptysis and shortness of breath. He was found to have pulmonary hemorrhage and was intubated on 6/6.

Systemic: Dizziness / Lightheadness-Medium, Systemic: Fainting / Unresponsive-Medium, Systemic: Vomiting-Mild, Additional Details: Patient briefly fainted post Covid vaccination, then vommited, stated that his head felt hot. Spoke to mother later in the evening. Patient felt much better.

Initially had headache and chills after vaccine, on day 3 after vaccine developed chest pain (midsternal and worse with inspiration), had one episode of emesis. On admission to hospital had elevated troponin, pro-BNP, Crp, normal EKG but echo consistent with Low normal left ventricular systolic function (biplane EF = 58.9 % and 3D EF = 55.37 %).

IMMEDIATELY AFTER PATIENT RECEIVED HIS VACCINE, HE FELT NAUSEOUS. PATIENT SAT DOWN AND SOON AFTER SITTING, STARTED SEIZING (HIS FATHER REPORTS ARMS CONTRACTING AND SHAKING). PATIENT DOES NOT REMEMBER THE EVENT. UPON REGAINING CONSCIOUSNESS, THE PATIENT SAID HIS FACE FELT FUNNY BUT THERE WAS NO THROAT SWELLING OR SIGNS OF ALLERGIC REACTION. PATIENT LAID DOWN WITH HIS FEET UP FOR ABOUT 15 MINUTES AND REPORTED FEELING MUCH BETTER. NO OTHER SIGNIFICANT EVENTS OCCURRED DURING THE OBSERVATION PERIOD. PATIENT DENIES A HISTORY OF PREVIOUS ALLERGIC REACTIONS OR REACTIONS TO VACCINES.

myocarditis based on chest pain and elevated troponin. also with chills, frontal headache, nausea and vomiting

Pt had walked to the lobby from the vaccination room and was sitting in a chair with his mask on. Five minutes after his injection with the COVID-19 vaccine he felt hot, nauseated, vomited a small amount of yellow emesis twice and became pale. He continued to sit up in the chair and a cool cloth was placed on the back on his neck and a fan was turned on. He was alert throughout. His BP was checked 4 minutes after the event and read 92/60 with a pulse of 74. He denied feeling lightheaded. He sat in the lobby with father by side and health department nurses monitoring him for 30 minutes. After the 30 minutes pt gets up out of chair without difficulty and ambulates around the lobby with steady gait and denies lightheadedness, nausea/vomiting or shortness of breath. Leaves accompanied by father. Encouraged to push fluid, stay out of the heat today and to take it easy the rest of the day. Checked on pt by phone through the father and he related that pt was feeling much better, denies symptoms.

Pt was given his 2nd dose of Pfizer vaccine at pharmacy on 6/3/21 at 1:20pm. Minutes after the vaccine pt developed headache, stiffness of neck, dizziness, fatigue and SOB. Epipen was brought out but never administered, pt was there for about 45 min. Was sent home and took Advil 400mg. Was playing badminton with lots of running 30-45 min prior to vaccine. No other vaccines given in the past year. Since the vaccine, pt has had fever up to 102. Continued headache, neck stiffness. 1st night 6/3/21- bad headache, throbbing headache all around head but worse back right side and then "spikes" on left side (like someone was hitting him with an axe) and fell asleep d/t sever pain, trouble breathing in the next few days 2nd night 6/4/21- neck pain and stiffness was really bad, he looked pale and almost disoriented. Had fever. Slow to respond. Would get rush of bad headache if he moved his neck too fast. Afraid he would fall down if he moved too fast. 3rd night 6/5/21 - (called 911) chest pain, difficulty breathing, vomited stomach acid, pain radiated down chest to the left, pain only when breathing in and went away the next AM 6/7/21 Chest still feels sore on Left side. Biggest issue right now is headache.

Abdominal pain and vomits, appendicitis diagnosed and surgery needed.; This is a spontaneous report from a contactable consumer (patient). A 15-year-old male patient received BNT162B2 (PFIZER-BIONTECH COVID-19 VACCINE), dose 1 via an unspecified route of administration, administered in Arm Left on 13May2021 13:00 (Batch/Lot Number: EW0179), at the age of 15 years old as 1st dose, single for COVID-19 immunization. The patient had no medical history and no known allergies. The patient's concomitant medications were not reported. The patient had no other vaccines in four weeks and no other medications in two weeks. The patient had abdominal pain and vomits, appendicitis was diagnosed and surgery (removal of appendix) was needed. The event occurred on 20May2021 04:00 and was hospitalized for 1 day. The patient underwent lab tests and procedures which included COVID test post vaccination nasal swab: negative on 20May2021. The outcome of the events was recovering.

Pt is a 15-year-old man with history of obesity, insulin resistance, mixed dyslipidemia, vitamin D deficiency, elevated hemoglobin A1c, anxiety disorder, and ADHD. He presented today to the ER with history of chest pain in the past 3 days. This chest pain is intermittent, sharp in nature, mainly in the retrosternal area, precipitated by deep breath, partially relieved by Tylenol, ibuprofen, or laying down. He denied any history of trauma, or recent viral illnesses. The severity of this chest pain ranging between 4?5/10 with no radiation. He had temperature of 38 °C on Saturday after the second dose of SARS-CoV-2 vaccine (on Thursday, 6/3/2021). He said that he had a loose bowel movement, intermittent cough, and headache. He denied any nausea, vomiting, nasal congestion, skin rashes, change in urine, muscle pain, or joint pain. At the ER, he was in stable condition, afebrile, with stable vital signs. He received 1 dose of Motrin 600 mg for the chest pain. ER dr. was consulted and he recommended admission to the CICU for further investigation and management. 6/9/21 - Troponin levels downtrending (last 1.30). ID consulted and recommended obtaining adenovirus PCR, enterovirus PCR, GI panel, Mycoplasma IgM, COVID IgG, COVID NP, and COVID spike protein. Will follow as an outpatient. CXR normal. EKG with sinus bradycardia, while calm. Echocardiogram results normal. RPP and COVID negative. Behavioral medicine followed. Discharged on 6/9/21. Hospital course CV Troponin level on admission was 2.7 and have been closely monitored. Last level obtained on 06/08 was 1.3. ECG 06/08 - sinus bradycardia. Echocardiogram 06/08 - essentially normal with trivial to mild tricuspid regurgitation and mitral regurgitation and trivial aortic regurgitation. Chest pain has now resolved and patient has good hemodynamics with good pulses, good blood pressures, and cap refill less than 2 secs. He will follow with Dr. in 1 week. No rigorous exercise for one week.

A contactable physician (parent) reported that a 15-year-old male patient received first dose of BNT162B2 (PFIZER-BIONTECH COVID-19 VACCINE), via an unspecified route of administration, administered in left arm on 22May2021 15:00 (Lot Number: EW0178, at the age of 15-year-old) as single dose for covid-19 immunisation. Medical history included seasonal allergies. Concomitant medications included loratadine (CLARITIN) taken for seasonal allergies; fluticasone propionate (FLONASE) taken for seasonal allergies. A pediatric gastroenterologist reported for his son. His son had the first dose of the Pfizer COVID-19 vaccine on 22May2021. On the next day on 23May2021, he experienced mild pain in the tongue and throat. He had no fever. The pain in the throat and tongue started on Sunday (23May2021), but it was mild and he was not concerned. His son had it on Monday (24May2021) and when he came home from school around 15:00 or 15:30, he was complaining of severe sore throat. His throat was hurting more. Severe from that time. He stayed home from school. Treated with Tylenol and Motrin. Didn't really work/helping. Pushing fluids. Felt pain when swallowing. He was not eating due to severity of the pain. Had severe pain when he tried to speak. He could not open his mouth well. Tried to keep his mouth closed. He could not talk normally cannot move tongue very much. They went to urgent care and had a strep test which came out negative on 25May2021. Reporter seriousness for pain in throat and tongue was disabling. The gastroenterologist wanted to know if his son's adverse reaction was an allergic reaction and if he should get the second dose of the vaccine. There was no prior vaccinations within 4 weeks. The outcome of the events was not recovered.

Developed fever, chills, nausea by about 9pm that night (6/12/21). Administered alternating tylenol/advil. Symptoms lasted approx 36 hours. Too weak and fevered to attend school the day following 2nd vax (6/13/21). Still fevered on the morning of the 14th, but given last dose of Tylenol and was then fine for school.

Patient became dizzy and clammy but did not lose consciousness. He began throwing up. No sign of anaphylaxis. Remained pale, dizzy, for about 5-10 minutes. EMT called to scene but was doing a lot better by the time they arrived. Treated patient with cold compress face/neck. Dad stated he has anxiety and that this same thing happened previously after having blood drawn. He was able to walk out on his own with his father approximately 30 minutes later.

Fever, nausea for 36-48 hrs in length. Cleared. 48 hrs later no fever but severe nausea, vomiting, and diarrhea for 36 hrs appxly. Has seemingly cleared currently for past 12 hours.

This is a 15 year old male without PMHx who presents with chest pain for one day after having Pfizer COVID vaccine 2-3 days prior. As per father patient had first dose without any sx except mild soreness on the site of injection. On 6/9/21 he received the second shot of Pfizer vaccine. That day he only fell mild pain on site of infection only. Next day he suffered from chills, HA, and fever. He took Tylenol for it. The day prior admission he had a mild headache and pain on the middle of the chest, dull, not radiating, and on/off. There was one episode of emesis after dinner and after taking TUMS he went to sleep. The parents check on him through the night and upon waking up he complaint still with chest pain, now increased and radiating to left side of the chest. The parents decided to bring him to ED for evaluation where he was found to have elevated troponins and he was admitted for myocarditis likely secondary to the Pfizer vaccine. His troponins were monitored q8hrs and he was started on Toradol q6hrs. See EKG and Echo results below. Due to increasing troponins 6/13, he was given IVIG with resulting improvement. At this time, we are still monitoring his troponin inpatient with plan for potential cardiac MRI. He will be discharged home on Ibuprofen 600mg TID and follow up with cardiology with exercise restriction likely for a few months, tbd by outpatient cardiology follow up.

Fifteen hours after the vaccine my son woke up and his respiratory rate was high. It was over 140. I woke him up and he had chills and diarrhea. Because of his Cortisol Deficiency I called his doctor. I was advised to give him 30mg Hydrocortisone and lots of water. His Respiratory and heart rate were high. He was feeling a little short of breath and he had heart palpitations. We went to the ER and they did lots of testing. He had a fever and Sepsis testing. All results were normal and they gave him a Liter of fluids. He was discharged after two hours and his pulse was still high at 105-110. We monitored him and his heart rate was slowing and he started to feel better. His Endocrinologist had him take a double dose of Hydrocortisone the next day. His improvement progressed and by the fourth day the symptoms were gone. He was able to do his normal activities but tired and worn out from the ordeal. One week later he had a slightly elevated heart rate but it resolved quickly. He had had a local reaction in his arm with swelling and a rash after the first vaccine.

Mom reports that nurse/vaccine administrator injected son/patient at a higher than usual site on the shoulder and the needle did not go in all the way, in fact of the needle was outside his skin. The skin around the needle puncture "bubbled" up and it caused him pain (more than the 1st dose when he received it). No leakage of vaccine fluid was observed. He had some soreness in the arm. The next day he did develop some nausea, body aches, and slight fever. He is feeling a bit better today (6/14) and only has some nausea remaining. Mom will take him to PCP to evaluate his Left shoulder since it is still achy.

The patient is a 15-year-old male with no past medical history who was in his normal state of health when he received a dose to the Pfizer COVID-19 vaccine on June 10th 2021. The day following vaccine he developed fevers and chills which she treated with Tylenol. On the morning of June 12 he developed chest pain and nausea which progressively worsened throughout the day. That day he presented to an outside hospital emergency department where he was found to have elevated troponin and EKG with ST segment elevation. He was then transferred to the hospital pediatric ICU for further care. On admission to the PICU treatment was started with IV Toradol. Echocardiogram was performed and revealed mildly decreased left ventricular systolic function. His peak troponin level was 27.43. He remains hemodynamically stable and was transferred to the pediatric floor. By 6/14/2021 his chest pain had resolved and he continued to improve with NSAID therapy alone. His troponin down trended to 2.62 on day of discharge. Repeat echo on day of discharge showed improving but still mildly diminished systolic function. Repeat ECG at discharge showed persistent ST elevation. He was discharged with a prescription to continue ibuprofen 600 mg 3 times daily and follow-up with pediatric cardiology in 1 to 2 weeks.

6/7 - patient received 2nd dose of Pfizer vaccine. 6/8 - patient developed tactile fevers (for 2-3 days), diarrhea (4-5 days of non-bloody diarrhea) 6/9 - developed chest pain and tightness and mild cough 6/12 - few episodes of NBNB emesis, cough with blood tinged sputum 6/13 - presented to urgent care, referred to hospital and admitted to floor. Patient found to have tachycardia and hypertension. CXR with pleural effusion, renal US with medical renal disease. Worsening renal function and concern for hyper coagulable state with worsening D-Dimer and possible pulmonary embolus but unable to obtain contrast images due to renal failure. 6/15 - transferred to ICU for heparin drip for worsening D-dimer. Nephrology, rheumatology and infectious disease consulted. Continued chest pain, cough with hemoptysis. Remains afebrile and otherwise hemodynamically stable. Started on ceftriaxone empirically. Concern for MIS-C, vaccine related adverse event vs autoimmune vasculitis.

Vomiting and pale after dose. Patient reported a history of vomiting in past immunizations. Gave patient water and monitored for 30 min. He was able to feel better and left the store feeling better.

C/O stomach cramps and went to rest room without relief. Siting up on table sipping water. Cramps still there however not any worse. School nurse in touch with parents and arranging for child to go home.

The night of vaccine, started to have nausea, lightheadedness, muscle aches. Progressed to neck pain and headache, and later to shortness of breath , chest tightness, and diaphoresis.

24 hours later, patient had severe abdominal pain, was found to have pancreatitis, confirmed by labs, CT scan and ultrasound scan, was admitted to Hospital. Very quick recovery, According to mother, he was given intravenous fluids, bowel rest, and nothing else.

He reports congestion and sore throat but reports no ear pain. He reports cough and difficulty breathing but reports no wheezing. He reports headache but reports no dizziness. He reports no nausea, no vomiting, and no diarrhea. He reports no sneezing and no runny nose. Fever with cough, mild SOB, fatigue and pharyngitis, likely viral illness. Ibuprofen 400 mg given here now. Negative COVID test here today, will send for chest x-ray, lab work CBC (H/H, RBC, indices, WBC, plt) C-reactive protein comprehensive metabolic panel lyme disease Ab w/refl to blot (IgG, IgM), troponin I, sed rate by modified westergren. EKG done, Continue to monitor for increased WOB or any worsening of symptoms. Consult with Cardiology, Echo recommended and pt. scheduled for one.

6/12/2021: Nausea, slight fever, dizziness, black outs, fatigue, muscle aches By night time, rashes started appearing all over his body including arms and legs and all over his face. The rashes looks like big blotches/ map looking red marks all over his whole body. His face was all covered with the blotches of rashes including his scalp. He had no fever though. He was breathing normally but the rashes were very itchy. gave him double dose of Benadryl. Then switched to adult Benadryl but still taking double the dose. This went on for 2 days and no sign of improvement despite taking Benadryl every 6 hours. Patient was sleeping most of the days due to the medication. 6/15/2021: reached to Family doctor and explained the situation. doctor recommended to go to ER after seeing pictures of the rashes. Patient adamantly refuses to go. Doctor suggested take Allegra on top of Benadryl. Patient took Allegra 24D. Patient mostly sleeping. 6/16/2021: saw some slight improvements. Back and chest cleared of the rashes but still heavy on the arms, most especially the thigh area and legs. still taking Benadryl but only the normal dosage. 6/17/2021: All rashes are gone and cleared/

A 15-years-old male patient received bnt162b2 (PFIZER-BIONTECH COVID-19 VACCINE, Formulation: Solution for injection, Lot/Batch number: unspecified, Pfizer NTEC), via an unspecified route of administration on an unspecified date as 2nd dose, single dose for covid-19 immunization (at the age of 15-years-old). Medical history included peanut allergy (he has a peanut allergy). The patient's concomitant medications were not reported. The patient historical vaccine included first dose of BNT162B2 for COVID-19 immunization. On an unspecified date in 2021, the patient received the second dose and at 5 o clock the next morning he vomited like 30 times, had a super fatigue, pale, achy, shaky fever of 101 degree Fahrenheit. He slept but was still feeling feverish. The clinical outcome for the events was unknown. Information on Lot/Batch number has been requested.

Diagnosis: Acute myopericarditis. Presentation: Pt had typical post-vaccine side effects on 6/7/21 including fever and malaise. Also had intermittent mild substernal chest pain starting 6/7/21. Malaise and fever resolved by 6/8/21 but intermittent chest pain continued to occur. Vomited x1 on 6/9/21 in early morning hours, which was followed by severe constant substernal chest pain, pain worse when lying down. Presented to medical care on 6/9/21, seen first via phone visit, then pediatric office visit, then sent to ED and admitted to hospital. Treatment: Naproxen 250 mg PO BID starting on 6/9/21, continues on this treatment as of 6/17/21. Outcome: Symptoms resolved on 6/9/21 after naproxen started, discharged home on 6/10/21. Doing well in outpatient without any complications.

Sore arm day of, continuing into day two. Woke at 9am day 2 feeling very achy and tired but with an upset stomach and shortly after vomited. He has a fever of around 101 and still has all other symptoms but stomach ache & nausea has subsided.

Approximately 11 minutes post COVID19 injection, client experienced syncopal episode per family member. Patient conscious and A+O x 4 at time of nurse response, client was noted to be pale and diaphoretic with c/o nausea, vital signs stable, lungs clear to auscultation bilaterally, no visible or verbal complaint of shortness of breath, edema to face/mouth, hoarseness/throat tightness, vital signs remained stable: 1351-HR 89, BP-92/64, SpO2-99%, R-18, 1353-HR 79, BP-102/74, SpO2-99%, R-18, 1400-HR 79, BP-108/78, SpO2-100%, R-18. Client waited an additional 15 minutes in clinic for observations, at time of discharge patient reported resolved nausea, skin color within normal limits for race, no visible or verbalized distress from patient.

Vaccine triggered increased migraines. In addition to vomiting, diarrhea, nausea, headaches, fatigue. Migraines have not improved much. Migraines before covid shot were 1-2 events per month. After covid they were daily for the first three weeks after first covid dose. Now they are 3-5 times per week! Had to put patient back on a daily migraine medication.

Started with chest pain and nausea 6/16/21 morning. Transported to Emergency Department due to recent news of possible vaccine side effects. Troponins found 11.47. Transported to Pediatric ICU for monitoring. Stable and resolved chest pain. Was monitored with peak troponin 44.96 on 2nd day of admission. Found to have myopericarditis, ECHO initially 56% ejection fraction on 6/16->55% on 67->62% on 6/22. Troponins downtrended after intravenous immune globulin given twice. Troponin 0.54 6/22 (repeat one pending) prior to discharge.

Patient is a 15 y.o. male presenting for 5 days of symptoms following his second dose of Pfizer SARS CoV 2 vaccine culminating in an urticarial rash which prompted him to present to emergency department. Patient received the second dose of the Pfizer vaccine on June 8 (first dose was administered on may 17 without symptoms). The next day (June 9) he complained of nausea and subjective fever. On June 10 he complained of chest "weakness" and shortness of breath intermittently. He had also mild light headedness but no syncope or palpitations. All those symptoms resolved by June 11. However, he had a diffuse rash on extremities, pruritic, and non-exudative.

Seconds after receiving the second dose of the Pfizer vaccine, patient complained of headache and stomachache and lightheadedness. Patient's eyes rolled and he became limp. Patient's mother and the pharmacist helped him to the floor, and he regained consciousness seconds later. He was given water and a granola bar and said he felt better after a few minutes. This adverse effect did not occur with the first dose.

days of left arm and 1 day of acute left side chest pain following 2nd administration of COVID-19 vaccine. Patient reports that 3 days ago, he received the 2nd COVID-19 vaccination (Pfizer ) in his left anterior deltoid area. Following this he developed diffuse body aches and headaches. On day prior to admission, patient developed fever at home with T-max 101.8° F. after going to sleep, he was awoken with nausea. Patient experienced episodes of vomiting, which he reported mostly consist of water, and also reported left arm and left chest pain at this time. He reports the pain felt like pressure and was a severity of 9/10. Patient also experienced numbness and tingling in his left hand. He attempted to take Tums and Motrin at home with mild relief of symptoms. Mother then brought patient in to Emergency Room. Patient denies syncope, no palpitations, pain does not worsen with deep breathing, pain unchanged by position.. emergency department course: Presented with temperature 99.7° F, heart rate 92, blood pressure 138/90, saturating 98% on room air. Laboratory work revealed elevated troponin, elevated CK-MB, and elevated creatine kinase. Chest x-ray was performed which was normal. EKG revealed normal sinus rhythm. Patient was transferred to our emergency room and subsequently admitted to the floor. Hospital Course: He was evaluated in Hospital Emergency Room and admitted to the floor for more close monitoring. His repeat labs showed elevated troponin and an echocardiogram was unremarkable. In view of the elevated troponin was case was discussed with the cardiologist who recommended a cardiac MRI which was performed which did reveal myocarditis, pericarditis. In view of this ICU service is consulted for admission closer monitoring further evaluation management.

Immediately following the vaccination, the patient told his father he felt dizzy, so he was directed to a chair. He told his father he could not see anything and was experiencing vertigo. His father then said his eyes and lips on the left side of his face seemed to be shaking as if he was having a seizure and he lost consciousness for a brief moment. When he opened his eyes, he told his father he was dizzy and his head hurt, but he could see again. I brought the patient some water to drink and took his first blood pressure reading at 10:42 am, which read 67/31, pulse 43. The patient mentioned that he hadn't eaten anything that day and that he was extremely nervous about getting the vaccination. His father got some orange juice and started to give him sips of it and he remained seated. The next blood pressure reading at 10:54 am was 115/59, pulse 76. The patient said that he was feeling nauseous, but less dizzy. I gave him two cookies to eat and took his blood pressure again at 11:01 am and it read 117/73, pulse 88. At this point, the patient said he was feeling much better and the cookies even made the nausea go away. The final blood pressure reading was at 11:13 am: 122/63, pulse 76. At this point, the said he felt completely better and father was confident in taking him home, so they left. I followed up with the father two days later on 6/25/21 and he said that he didn?t have any more problems after they left the store. That evening, patient's temperature went up to 99 and his head hurt with movement, but otherwise, he was fine and did not have any more symptoms after that.

Paramedic stated to pharmacist that patient did not show signs of being post-seizure. Patient was put on strecther by paramedics

Acute appendicitis; This is a spontaneous report from a contactable consumer (patient). A 15-year-old male patient received second dose bnt162b2 (PFIZER-BIONTECH COVID-19 VACCINE, Solution for injection, Lot Number: EW0187, Expiration date not reported), via an unspecified route of administration in Arm Left on 15Jun2021 13:15 as dose 2, single for covid-19 immunisation administered at Doctor's office/urgent care. Medical history and concomitant medications were not reported. No known allergies. Previously the patient received first dose bnt162b2 (Lot number RW0185), in Arm left on 25May2021 13:15 for covid-19 immunization. On 16Jun2021 20:00, the patient experienced acute appendicitis and was admitted in emergency room/department or urgent care and underwent appendectomy as a corrective measure. The patient did not receive any other vaccines within 4 weeks prior to the COVID vaccine. Prior to vaccination, the patient was not diagnosed with COVID-19; Since the vaccination, the patient not been tested for COVID-19. The outcome of the event was recovering. Information on Lot/Batch number was available. Additional information has been requested.

Developed chills, nausea, headache, vomiting on 6/24/21-6/25/21, then developed palpitations 6/26/21 prompting evaluation in emergency department and hospitalization for myocarditis

Patient taken by mother to ER on 6/20/2021 for diarrhea and abdominal pain which had started on 6/14/2021. Received 2nd covid vaccine at 4pm on 6/13/2021. Was well until 6/14/2021. Diarrhea and abdominal pain started evening of 6/14. Vomited twice on 6/15. Watery loose non bloody diarrhea. RLQ Pain rated as 7/10. No fevers, cough, congestion or rashes. At ER: US inconclusive, CT showed ascending colitis and possible ileitis. Appendix wnl. Received flagyl, ceftriaxone, toradol and morphine in ER Transferred to Medical Center on 6/20 for hospitalization for pain control, IVF and GI consult. Received toradol and morphine each x 1. Started on hyocyamine. Discharged from Hospital evening of 6/21/2021. Seen in my office 6/23/2021. Stool and appetite wnl. RLQ pain rated as 3/10.

At 1 am, my son presented with headache, dizziness, lightheadedness, shaking, ringing in ears, shortness of breath, and pallor. Vasovagal symptoms and near syncopal episode. Was able to get him to the floor to avoid a fall. Vomited several times. Severe vasovagal symptoms began to resolve within one hour. Didn?t check heart rate during initial near syncopal episode but was tachycardic after initial stabilization. Headache and fatigue persisted and is still lingering today.

The patient has a known genetic predisposition to pancreatitis. He received his first Pfizer Covid vaccine on 5/23. On 6/13 he had mild pain consistent with his history of pancreatitis. He was able to treat this at home with oral pain medications so did not have to seek other care. He received his 2nd Pfizer Covid vaccine 6/19. On 6/24 he had onset of more severe abdominal pain so went to the ER. There his lipase was found to be >6000. He was admitted for acute pancreatitis. He was given IV fluids, pain control until his symptoms resolved.

Systemic: Fainting / Unresponsive-Medium, Additional Details: patient fainted 4 minutes after receiving vaccine. Patient said he did not eat or drink before getting vaccine. blood pressure 105/90. he recuperate quickly but mother wanted us to call 911 to check him. He was responsive and moving. episode only last 3 minutes. i gave him a bottle of water and he felt fine but tired.

Sweating, headache, nausea, light headedness, blacked out, dizzy, body aches - after second dose. Tried Ibuprofen so far. Waiting to see if there is relief. Can barely stand on his own.

Had vaccine at 4 pm on 6/23/21 1 am woke to racing heart and palpitation lasted two hours and resolved 6 am woke fever chills and achey 10 am racing heart, palpitations then resolved 1pm severe chest pain, severe tachycardia and low BP Went to ER continued to be tachycardic and low BP, chest pain less, had episode of HR shooting up to 180 and then violently throwing up cardiac labs and EKG were normal left with tachycardia on standing and suggested we follow up with cardiologist asap- appt is 7/6/21 received one bag of IV fluids fever, erratic heart rate and lethargy resolved on 6/27/21

Child diagnosed with appendicitis.; This is a spontaneous report from a contactable consumer (patient). A 15 years-old male patient received 2nd dose of covid vaccine lot number=EW0180, at the age of 15 years old, on 08Jun2021 04:00 PM on Left arm for covid-19 immunization. No known allergy. Patient did not receive any other vaccines within 4 weeks prior to the COVID vaccine. Other medications the patient received within 2 weeks of vaccination was multivitamin. Historical vaccine was the 1st dose of BNT162B2 (lot number=EW0177) on 18May2021 04:00 PM on Left arm for covid-19 immunization. No reaction on previous exposure to vaccine. On 14Jun2021 11:00 PM, Child diagnosed with appendicitis. AE resulted in Emergency room/department or urgent care, Hospitalization 1 day. Treatment was Appendectomy. Seriousness criteria-Caused/prolonged hospitalization. Prior to vaccination, patient was not diagnosed with COVID-19. Covid test post vaccination: covid test type post vaccination=Blood test on 15Jun2021, Negative. Outcome of the event was unknown. No follow-up attempts are needed. No further information is expected.

Patient Vomited about 10 minutes after the Pfizer COVID-19 vaccine. We gave the mother a water bottle, asked how he was and he said he felt better. About 10 minutes later they left without checking back in with the staff. The RPh on duty was watching him for other reactions and he seemed fine.

7/4/2021: 15 year old male with autism and ADHD that presents to PICU from another hospital status post cardiac arrest times two. Mom states that patient has been having coughing for the past 2 weeks. Went to PMD on 6/25 who noticed wheezing on his exam. Ordered a CXR for 6/29. The next day, told mother of the results which showed pneumonia, "fluid in lungs." Started on albuterol q4, cefdinir, and OTC cough medicine with no improvement of symptoms. Began having decreased appetite, urine output. Had a couple episodes of post tussive emesis with blood tinged secretions. Abdominal pain started the day before admission. Patient continued having increased coughing and shortness of breath. On day of admission, patient was walking up the stairs to take a shower. He then fell and hit his head on the wall. Denies LOC at that time. Stated that he could not breath. Patient brought to ER. Patient went into cardiac arrest at 1840 until 1910. CPR was done, given epinephrine, and intubated. Then went into cardiac rest again. Placed on Vent, TV 450, PEEP 6, Rate 15, Peak pressure high 30s. Given ceftriaxone, vancomycin, toradol, 40mg lasix, 4mg zofran and 2L NS bolus. CBC, CMP, blood gas, troponin, pro BNP, RVP/COVID ordered. Started on versed (6.5), epinephrine (20) and norepinephrine (10) drips. Was then flown by helicopter to our PICU. Of note, patient had second dose of Pfizer vaccine on 6/19. Mom believes symptoms started just before his vaccine dose.

Also received 1st dose COVID19 (Pfizer) at same place. Administered on 6/12/21. Lot number of first vaccine EW0182. Patient developed acute chest pain on evening of 7/5. Had some shortness of breath and vomiting. Brought to ER. Troponin was found to be elevated at 3238.9 (normal range 0-54). COVID-19 PCR and respiratory viral panels were negative. Admitted to ICU for observation. In morning, feeling much better. Troponin down trending. Echo was normal.

Nausea, stomach cramping and vomiting and dry heaving for several hours. Fatigue, body aches, fever of 100.7. Tylenol and ibuprofen given every 4-6 hours. 30 hours or so of feeling unwell.

the patient was a 15 year old male , came in for the first dose of pfizer vaccine, after getting the vaccine , the patient suddenly fainted for a moment, but came back in less than a minute, he was still little dizzy for couple minutes after that, but was perfectly ok after 5 minutes, he also threw up, no other allergic reactions noticed, . mom had mentioned that he hadn't had anything to eat in the morning and he was really nervous too, that might be the reason she said. but we did monitor the patient for over 30 minutes and he was perfectly fine before he left

Abdominal pain coming from the right testicle, starting at 5:30 AM, ~14 hours after vaccination. The pain made it difficult to just to lay in bed. 5/10 pain. I took Tylenol later in the morning and the pain subsided.

Systemic: Dizziness / Lightheadness-Medium, Systemic: Nausea-Medium, Additional Details: Patient's blood pressure was 114/80 pulse 68. Symptoms resolved after 15 minutes.

He felt dizzy 3-4 minutes after he received the shot. He vomited 7 minutes after receiving the injection. At 4 minutes after vomiting he felt better. He drank water and had cool wet clothes on his forehead and neck. He remained seated at our pharmacy department until he felt completely better. He left accompanied by his mother.

. Patient developed fever, malaise and loss of appetite. Within a couple of hours of receiving his second dose. On 07/11/2021 patient woke with severe chest pain and shortness of breath. Presented to the emergency room. Found to have sinus tachycardia with nonspecific ST and T-wave changes on EKG, normal troponin, but elevated d-dimer and CRP. CBC within expected limits. Chemistries basically normal. CT scan of the chest with PE study negative for PE

Patient admitted fur to chest pain and dyspnea and found to have myopericarditis after 2nd dose of COVID-19 Pzifer vaccine. Vaccine was administered at pharmacy. First dose received on 5/15/2021, second dose on 6/5/2021. Fatigue and chills started a few hours after second dose and then developed shortness of breath, nausea, emesis and 8/10 midsternal chest pain. Presented to an urgent care facility where CXR and EKG were obtained. EKG showed early repolarizations. Labs were obtained at a local ED and showed elevated troponins, initially 11k and then to 20k and peaked there. BNP was within normal limits. Subsequent EKG showed diffuse ST-segment elevations concerning for acute pericarditis. No prior infections or symptoms like this previously. He was given colchicine at outside facility prior to transfer. At hospital he was continued on ibuprofen and monitored with telemetry and had ECHO and cardiac MRI. Latter was consistent with myopericarditis. ECHO showed normal function. He received PPI while on NSAIDs. His symptoms resolved and he was discharged to home on 6/10/2021 with activity restrictions and plan to follow-up with peds cardiology. Of note, there was an error submitting the initial VAERS report for this patient during the admission, therefore, this delayed report is being submitted.

The patient was given the first dose of the Pfizer covid vaccine, subsequently he was standing waiting with his mother for his younger sibling to receive his vaccine and after I had administered that, I turned around and saw that he was starting to lean over and his mother and I went to grab him as he went down and he fainted momentarily. We slowed his descent, but he had a small abrasion on his head (I think from the carpet). He said he felt dizzy and nauseous. I'm not sure if he was also watching while I was giving his sibling the shot and that exacerbated the situation. We had him checked out by EMT and he was fine after the initial fainting.

patient received the vaccine. patient was just fine for like 10-15 mins or so. When it was about time for them to leave, he stood up and thats when his mother called for help that he wasnt feeling well. he laid down on the floor for a little bit but then sat back up and sat back on the chair and said he was feeling better. i asked if they wanted us to call 911 and the mom said no. then, the patient said he had to vomit so we asked where he would like to vomit the bathroom is right next to him. patient chose to walk to bathroom and threw up a couple times. patient felt more dizzy/faint in the bathroom so the mother said please call 911 so we did. we sat with patient sitting on a chair for a couple mins and offered little cup of water. tried to talk to the patient and see if he was conscious. patient was responding to questions mom asked in another language. then, the paramedics arrived and put patient on a stretcher and said he seemed ok but was going to take him to hospital just in case. patient was breathing fine and not shaking.

Patient presented to the ED with 10/10 chest pain. Two days prior, he'd gotten his second COVID-19 pfizer vaccine. He denied any fever, congestion, cough, vomiting, diarrhea, or other signs of infection recently; no sick contacts. On arrival to the ED, he had normal vital signs, but physical exam was notable for sternal/epigastric pain. Laboratory evaluation, including CBC, CMP, CRP, urinalysis, urine culture, high sensitivity troponins, and lipase were obtained and notable for HS troponin of 303 pg/mL with a 2 hour repeat of 291 pg/mL. CRP was 3.5 mg/dL. His EKG had diffuse ST elevations. CXR was obtained with no evidence of cardiac border or consolidation. RUQ ultrasound showed no evidence of cholecystitis or other abnormality. Patient was admitted for presumed COVID-19 vaccine-induced myopericarditis. Echocardiogram showed normal biventricular function and no pericardial infusion. He was treated with IV toradol, with resolution of chest pain. HS troponins reached the 400s, with downtrend to the 300s prior to discharge. He was discharged on PO ibuprofen with follow-up cardiac MRI and labs.

On June 2, 2021, he awoke and complained of feeling hot, tired, and not hungry. His mother had been sick with acute gastroenteritis symptoms 2 days earlier. Over the course of the day, he had a fever to 101.6 F, and began to have loose stools (twice). He received Ibuprofen 400 mg by mouth every 6 hours for the fever, and drank plenty of fluids. On June 3, 2021, he continued to have fevers to 101.5 F, loose stools (twice again), and poor oral intake of food. I canceled his scheduled 2nd Pfizer-BioNTech vaccine, and got him a NP swab test for SARS-CoV-2 through the health department. On June 4, 2021, he had no fever but now complained of a rubbing left parasternal chest pain. On physical examination, his BP was elevated (130s/90s), he had a slight tachycardia (90s), and was afebrile. On auscultation, I heard a S4 gallop and took him immediately to the local ER.

A 15-years-old male patient received second dose of bnt162b2 (Pfizer-BioNTech Covid-19 mRNA Vaccine, Solution for injection) via an unspecified route of administration, administered in Arm Left on 19Jun2021 10:00 (Batch/Lot Number: EW0217; Expiration Date: 30Sep2021) as single dose (at age of 15-years old) for covid-19 immunization. Medical history included Allergy to penicillin (10 years ago). The patient previously received first dose of bnt162b2, solution for injection administered on 29May2021 sometime around the morning in left arm (Lot Number: EW0180 and Expiration Date: 31Aug2021) for COVID-19 immunization. There were no concomitant medications. No additional vaccines administered on same date of the Pfizer Suspect. No relevant tests done. No prior vaccinations (within 4 weeks). It was reported that, on 19Jun2021 around 06:00, migraine which began at first on the day he received the second dose of the covid vaccine, an hour or so later. Then it came back again today. Until about 10:00 it was the same, then it got better and on the same day the patient experienced pretty bad headache. On 22Jun2021 06:45, about 45 minutes afterwards he was pretty nauseous, it got a little bit worse between 8:00 and 10:00, but currently he was better. Caller's mother was also on the line. She asked, what stage should we got the doctor and wanted to know how long the side effects might last and when they should be concerned. The patient did not visit to emergency room or physician office due to adverse events. The outcome for event headache was not recovered, for nausea and migraine it was recovering. No PQC was involved. Information on the lot/batch was available, Additional information has been requested.

PT ADVISED THAT HE VOMITTED FROM HIS LAST COVID19 VACCINE. PT HAD THE SAME REACTION WITH THE 2ND DOSE FROM THE PFIZER COVID 19 VACCINE. PT VOMITED X 2.

ER visit with severe abdominal pain and vomiting. Admitted to hospital due to high liver enzymes and lymph node activity. Follow up exams including extensive labs, CT, MRI and ultrasound point to autoimmune hepatitis.

Patient passed out around 5 minutes past dose. He did not have a complete loss of consciousness. Lowered him to floor, elevated legs, and slowly lowered head to ground. complained of being cold, had shivers, and complained of ringing in the ears and nausea. once resolved, sat him up and observed again. around 30 minutes past vaccine had a second fainting spell, to which called 911 for observation. Patient completely resolved and left with mom and EMS walked him to her car.

Pt had severe anxiety and claustrophobia prior to vaccine. Immediately following administration patient went limp/fainted, awoke seconds later was nauseous and vomited slightly. Patient was pale. Asked patients mother if he had eaten today he had not. Provided patient with a iced tea he brought with him. Upon sitting, monitoring temp, talking with patient and mom, patients color returned and patient was feeling better. Patient remained seated for approximately 30 minutes and upon leaving patient was feeling much better.

Patient had a vagal response caused by an anxiety attack due to fear of needles. Technician, gave the vaccine and then immediately called me in. The mother and son were worried about the needle disappearing due to the retraction mechanism. The son was loosing color in his face and becoming dizzy. I had him sit upright, talked with him for a few minutes, gave him Benadryl as a precaution, and checked his blood pressure. He was having no difficulty breathing and blood pressure was WNL. His mother said that he has passed out with needles before and that he had not eaten today. Eventually the patient claimed to lose his sight, and within 30s passed out. He was still sitting in the chair, bent over, vomited, and then immediately came back to consciousness. The patient then said that he felt fine and wanted to leave. I talked with him and his mother for around 20 minutes, had him eat a banana, and drink a full bottle of water. After the time, he felt fine and him and his mother left.

2 weeks of abdominal pain & nausea, documented weight loss of 10lb, prolonged malaise limiting his scholastic learning, all associated with having the first covid-19 (pfizer) vaccine on 6/7/21 He had fever lasting several days that began within 2 days of vaccination (but that resolved within several day ). Labwork done at that time showed modest nonspecific elevations of inflammatory markers CRP of 42, (normal < 8), ESR 50, calprotectin 285 (120 limit). SARS covid-19 antibody semiquant to spike protein RBD was 64.4 U/mL (<0.8, Labcorp). Other tests did not identify another specific etiology, and an evaluation by another physician for myocarditis was reportedly normal. As of 7/17 he feels better with a resolution of all symptoms, labwork repeated this past week (7/15) showed a resolution of those same inflammatory markers to normal levels .

A 15-year-old male patient received BNT162B2 (PFIZER-BIONTECH COVID-19 VACCINE), via an unspecified route of administration on 18Jun2021 (Lot number and expiration date was not reported) as dose 1, single for COVID-19 immunization at the age of 15-year-old. The patient medical history and concomitant medications were not reported. The reporter wanted to know if it was possible to get the vaccine one day other than 21 days and getting exactly on 20 days instead of 21 days. The patient experienced little bit nauseous, it was not major on an unspecified date in Jun2021. No treatment was received for the event. The outcome of event was unknown. No follow-up attempts are possible; information about lot/batch number cannot be obtained.

Temperature 104.0, headache, stomach ache, chills - 0400 Headache started earlier in the night approx 0200

Patient had flu like symptoms that developed the evening of vaccination including fever, headache, myalgias, nausea and vomiting. He had a syncopal episode on 7/18 (2 days after vaccination) and family found him on the ground. Following this episode he complained of substernal pleuritic chest pain. Initial EKG demonstrated diffuse ST changes. He became hypotensive and developed respiratory distress with evidence of pulmonary edema on CXR. He was admitted to the PICU on 35L HFNC and received an epinephrine drip for blood pressure control. Respiratory status improved.

My child was complaining of the chest pain two days after the first Pfizer vaccine. Had vomiting, diarrhea and slight fever one day after the vaccine. All symptoms resolved after three days.

Medical history included ongoing known allergy in mosquito bite (sketter syndrome) an allergic reaction to mosquito bites, as his son has grown up it has become less and less relevant. Concomitant medications were not reported. Patient previously received first dose of bnt162b2 (Dose:01 Lot number: EW0171 Anatomical Location: Arm left) on 17May2021 for COVID-19 immunization. It was unknown if the patient received any other vaccines within 4 weeks prior to the COVID vaccine. Prior to vaccination, was the patient was not diagnosed with COVID-19. Since the vaccination, the patient been tested for COVID-19. History of all previous immunization with the Pfizer vaccine considered as suspect (or patient age at first and subsequent immunizations dates of birth or immunizations are not available. It was reported that my son traveled on 16May2021. Covid test prior traveling negative. On Jun2021, 4 days after vaccination develop GI, heavier insomia, dierrea, vomiting, fever, hypotensive, dehydraeted. On 11Jun2021 experienced pericardial effusion. On an unknown date Jun2021, developed into MISC and was intubated. The patient was hospitalized from 14 Jun 2021 to 6 Jul 2021. Treatment was received. On 15Jun2021, admission at (withheld), (Hidratation IV and IVIGs). On 16Jun2021 condition worsen and Intubated in (withheld). On 17Jun2021, transfer to (withheld) (7 days at PICU, another 7 at pediatrics). On Jun2021, developed a strong insomnia and headache. Events resulted in emergency room/department or urgent care. Treatment received for the adverse event was MISC protocol IVIGsAnakinra, esteroids and others. On an unknown date, Patient said he had 4 vaccines. His son spent 7 days in the ICU and 7 days in the pediatric unit was discharged from the hospital yesterday. Reported that other researchers studying MIS-C in children have seen how the epithelium cells in the intestine become loose, then the protein goes through the blood stream and can start a cytokine storm and asked if we have studies regarding how much S protein is being produced by different age groups; can it be checked how much S protein is going into the bloodstream to see adverse effects of the vaccine and stated he needs more data to see if his son was possibly infected with Covid during his vaccination period or what he previously infected and absolutely asymptomatic. Most MIS-C occurs 4-6 weeks after having Covid. States that for his son to travel he had to have antigen testing, which was negative. His son did well until 3-4 days after the second shot. States at that time his son became ill with vomiting, diarrhea, headache, and insomnia. She took her son to the hospital and they found him to be hypotensive and dehydrated. They started intravenous fluids and their protocols; the hospital did not have all the required medications for their son and recommended he be transferred to (withheld) Hospital. He was very tired and they decided to put him on ventilation for the ambulance drive. States that when they arrived in (withheld), his son was placed in ICU where he was on the ventilator for 5 days, the recuperating for 2 more days. His son was then moved throughout the hospital system until his discharge yesterday. His son's insomnia may have started before he left for the (withheld). States that it was worsened after the vaccine, his son mentioned after his hospitalization that he was actually given 4 shots the day he got his vaccines, assumed it to be some kind of delirium from his son being in the ICU for so long, stated he told his son that was not possible that he got 4 vaccines at once because of the paperwork you have to fill out to get the vaccine, the lady at the vaccination site told him to sit down and then gave shothim four shots. The patient underwent lab tests and procedures which included IgG and IgM resulted as positive on 15Jun2021, PCR resulted as negative on 15Jun2021 Nasal Swab, PCR resulted as positive on 17Jun2021 Nasal Swab. The clinical outcome of the event developed MIS-C and was intubated, vomiting, dierrea, headache, insomnia, hypotensive, dehydrated on an unknown date 2021 was resolved and the event pericardial effusion, develop GI, fever was resolving, the event condition worsen, Vaccination failure, patient said he had 4 vaccines and PCR resulted as positive on 17Jun2021 Nasal Swab was unknown; Sender's Comments: Based on the limited information currently available, the causal association between the event Insomnia, Diarrhea, vomiting, fever, Drug ineffective, COVID-19, Headache, overdose and the suspect drug cannot be excluded. Also, there is limited information in the case provided, the causal association between the event Multisystem inflammatory syndrome in children, Pericardial effusion, Gastrointestinal inflammation, Condition worsened, Hypotensive, Dehydration and the suspect drug cannot be excluded. The impact of this report on the benefit/risk profile of the Pfizer product is evaluated as part of Pfizer procedures for safety evaluation, including the review and analysis of aggregate data for adverse events. Any safety concern identified as part of this review, as well as any appropriate action in response, will be promptly notified to RAs, Ethics Committees, and investigators, as appropriate.

7/9/21 developed moderately severe throbbing headache localized to his forehead with moderately sever painful "tight" sensation in the muscles of the back of his neck and lower back. Pain persisted for ~ 3 hours ; Temp 101 Tylenol l taken but fever and pain returned after 3-4 hours. 7/10 awakened with nausea, dizzy/lightheaded , headache, vomiting. symptoms continued until 7/12/21. Viral infection suspected but nonetheless prescribed cephalexin "in case". Fever and headache continued with once daily eesis so came to ED for evaluation 7/14/21

Patient presented 3 weeks after the vaccine with worsening dyspnea on exertion, orthopnea, cough that began 2 days after vaccine administration. Two days prior to presentation to Harbor ER on 7/1/2021 he had nausea and vomiting with a fever 101F on day of presentation. He was found to be in congestive heart failure with severely diminished biventricular function. While inpatient he had worsening respiratory status requiring CPAP and O2 though he improved with initiation of diuresis. His inflammatory markers were not elevated and he had negative troponins. cMRI revealed an LVEF 13% with no signs of active myocarditis. Viral studies were negative and there was no evidence of bacterial infection. Prior to this, he was in his USOH with no limitations. He was seen in cardiology clinic 5/2021 and was stable at that time. He required initiation of diuretics and oral heart failure management and was discharged with close follow-up.

Patient experienced vasovagal symptoms, blacked out, excessive sweating, difficulty swallowing/drinking, difficulty keeping head up. Symptoms occurred within a few minutes of the vaccine injection. Patient stated similar symptoms when they drew blood previously. Patient started feeling better after a couple a minutes sitting down.

Received 1st dose of Pfizer Covid Vaccine and started becoming increasingly manic within about an hour. Complained of stomach discomfort. Became increasingly anxious and manic and was only able to sleep for two hours (received at 4pm, slept from 12am to 2am). Awake and manic the day following. Complained of stomach pain. Eating normally with no other symptoms. Started complaining of being cold and chills. Applied rotating heated blankets until he feel asleep about 10pm. Woke up at 2am. Manic and anxious. Stomach started swelling more and becoming rigid. Gave him Miralax. No change in swelling. Increased stomach pain. Gave him a suppository. Immediate small bowel movement. After two more hours and no change in swelling, gave a second suppository. Immediate small bowel movement. Stomach becoming distended and complaining of increased pain. Took him to the ER. Pain amplified on the way. Admitted to PICU after EKG in ER. Quickly declined and went into respiratory failure. Right lung had no air exchange and left lung collapsed. Myocarditis, pleural effusion, kidney damage and new onset Type I diabetes. Unexplained pneumonia. No growth from fluid in lung.

Patient lost consciousness two times, one on 06/28 and the second one on 07/15. First time patient felt dizzy/nauseous, blurred vision (loss of color), loss of hearing, and then loss of consciousness. The patient woke up 5 minutes after. Patient felt slight confusion with no adverse effects after. Second time patient almost lost consciousness three times before finally losing consciousness. First time patient loses consciousness for less than 3 seconds before waking up. 3 minutes later patient felt dizzy before losing consciousness for less than 3 seconds. 6 minutes later patient felt dizzy before losing consciousness for less than 3 seconds. Less than 1 minute later patient felt dizzy and nauseous before losing consciousness for 1 minute. After waking up patient felt slight confusion with no adverse effects.

Patient is a 15 year old M with hypofibrinogenemia, platelet aggregation disorder and history of COVID admitted to Inpatient Ward from 26 June - 01 July for work-up and management of new onset and severe headache, photophobia, and pain with extraocular movement. Extensive work-up complete but still unclear etiology, though near complete resolution of symptoms with IV steroids. Neuro: CSF positive for HHV-6, still unclear if this fully explains presentation. CT and brain MRI negative, though limited exam due to braces artifact. Headache complicated by clear post-LP postural headache, though this improved throughout hospitalization, with nausea moderately controlled on Benadryl. Neurology and Ophthalmology consulted, who both thought peri-neuritis was plausible, which is consistent given such rapid improvement with steroids. Pain moderately controlled with Tylenol. Oligoclonal bands and MBP CSF studies pending at time of discharge. Plan for 7-day steroid taper as an outpatient (outlined below in patient instructions), and Neurology follow-up in about 1 month. ID: Initially empirically covered with Ceftriaxone and Acyclovir, but once negative CSF, both were discontinued. He remained afebrile throughout remainder of hospitalization, so no further antibiotics started. Had remote history of immunodeficiency (confirmed here to be IgA deficiency), underwent more extensive immune work-up with lymphocyte subset 6 pending at time of discharge. Fen/GI: Nausea and poor PO intake secondary to headache and neurologic symptoms. Unremarkable KUB with no focal findings on exam. Tolerated good liquid PO intake prior to discharge, improving food intake with help of Benadryl and Reglan (Zofran did not help his nausea). Given risk of Reglan side effects, trial of Benadryl alone was given and relieved his nausea. Will discharge with Benadryl PRN for nausea, though expect this to improve. Heme: No blood transfusions required, no history of recent bleeding/trauma, no signs of active bleeding. Received TXA for three days following his LP performed on 6/28. Rheumatology: Patient has preexisting appointment with Rheumatology scheduled on 7/9 due to a history of intermittent joint pain/swelling with skin color changes since April 2020. Ordered requested Rheumatology labs by Doctor while patient is admitted. CRP and ESR resulted normal at 0.31 and 8 (respectively), C3 and C4 within normal limits, dsDNA and ANA negative. Repeat ESR on 6/30 also normal at 10. Pending labs include fecal procalcitonin. Follow up appointment with Rheumatology on 7/9. CV: Some concern for myocarditis at time of presentation given pleuritic chest pain, history of COVID-19 infection, and recent COVID-19 vaccination. EKG at that time revealed nonspecific T wave changes. However, troponin and BNP resulted negative and patient's chest pain has resolved at this time. Discussed with Cardiology who did not think further work-up was necessary. Chest pain completely resolved at time of discharge.

15 yo male presents with mother to receive 1st vaccination. After being observed for 15 minutes, the patient and his mother left to walk to the car. Upon walking to the car, patient vomited 3x and was escorted back inside by EMS. Patient stated he had a headache immediately following vaccine. Upon re-entry into the building, patient was given juice and water and started to feel better. Upon interviewing the mother, she stated the patient came from a 3 hour long football practice this AM and barely slept last night. Mother states she thinks the vomiting "was more from heat exhaustion" than the vaccine. Patient remained monitored by EMS until he felt better, patient left with mother after symptoms resolved.

Patient received his 1st COVID (Pfizer) vaccine and he bled more than usual. Patient reported feeling faint and nauseous. While monitoring patient, he threw up a few minutes later. He felt better shortly after. We kept in the pharmacy area for observation for 30 minutes to ensure that he was ok before he left. Patient was feeling fine when he finally left.

I don't know if any of his adverse events are related I wanted the CDC to be aware. He started feeling uneasy, unwell, vomiting, itchy ears (both), upper chest pain, upper back pain and difficulty breathing. We went to the ER at 17:00 - 22:00 on 7/18 was discharged couldn't figure out what was happening was given medication for migraine, Benadryl, Loratadine.

3 days of constipation, feverish, fluid in stomach, developed an abscess, healthcare professional decides to remove his appendix.
