The patients legs appeared to go rigid and he make movements with his arms. He appeared very pale. The patient "came to" within approximately a minute? Patients mother and I were able to lay Patient down on the floor. I asked his mother if she would like us to call 911 and she said yes. Prior to the incident, I discussed Patients past history of seizures with his mother. She sail he had seizures when he was younger and had been on medication, but he was no longer on any medication and had been seizure free for approximately 5 years.

ED Note 5/19/21 Patient is a 12-year-old male with hypogammaglobulinemia, asthma, IBS who presents with weakness in his lower extremities. 6 days ago, patient had seafood and later that night vomited once, nonbloody, nonbilious. 4 days ago, he received the first dose of the Pfizer SARS-CoV2 vaccination. That day, he also felt some burning over the anterior parts of his bilateral ankles. Over the past 3 days, he has been sleeping a lot and had a headache. He has not had a fever or other muscle aches, no subsequent N/V, abdominal pain. Last night, he reported ankle pain to his mom. Today at school, he was playing kickball when he had the sensation that his ankles were not working/gave out and he fell to the ground. He had difficulty getting up and required a wheelchair. He did not lose consciousness, denied CP, palpitations, SOB, headache, vision change during or before that episode. He was not incontinent of urine/stool and had no abnormal movements noted at the time. Since then he has noted twitching in his thighs, calves and toes. He reports weakness throughout both lower extremities and has difficulty with intentional movements at the toes/ankles/knees, weakness in hips. He reports a burning sensation over the anterior ankles/dorsum of foot b/l. Denies fevers, chills, cough, SOB, CP, palpitations, abdominal pain, N/V/C. He endorses diarrhea 3x daily which is his baseline. He has panic attacks 3x daily w palpitations/SOB that self resolve. He has been on keflex for an ingrown toe nail for the last week. MRI performed with normal brain and spine. However, on repeat exam patient has diminished sensation in the bilateral feet and diminished ability to dorsiflex or plantarflex both ankles with very limited toe range of motion. Still with preserved reflexes in the patella and Achilles. Discussed at length with neurology and the family. Differential diagnosis at this point of functional neurological disorder versus Guillaine Barré syndrome. Neurology note 5/21/21 Patient is a 12yo M with hypogammaglobinemia, IBS, and poorly controlled anxiety, presenting with 4 days of lower extremity paresthesias and 1 day of hyperacute onset of lower extremity weakness and perhaps some mild confusion. History notable for covid vaccine three days prior to presentation and anixety surrounding covid and return to school. His general examination is notable for anxious affect with difficult to localize neurologic exam. Exam shows possible bilateral lower extremity weakness of TA and hamstring>quad/IP that is very challenging to grade due to poor effort and giveway, and decreased sensation in a bilateral stocking/glove distribution to all sensory modalities. MRI w/wo contrast brain and spinal cord normal on admission. LP perfromed in ED also normal. Given negative workup and history of anxiety with recent psychosocial stressors, most likely sudden onset weakness is secondary to a functional neurologic disorder. Plan for discharge to rehab pending PT evaluation.

The day prior to admission, he developed a diffuse popular rash. He was seen in clinic on 5/20 and lab work was obtained, which was consistent with MIS-C findings (elevated troponin, elevated fibrinogen, elevated CRP, hyponatremia, relatively low absolute lymphocyte count).

We called a code white in the store had management as well as pharmacist offer ambulance to patient and also monitored the patient. Mother and son left AMA and went to call patient's doctor.

Chest pain every 20-30 minutes, worse when supine, pain with breathing when laying down that resolved when sitting up, presented to ED for workup. Found to be tachycardia to 100s, hemodynamically stable otherwise. Pro-BNP 193, CRP 1.57, Total bilirubin 1.1, d-dimer 274, troponin initially normal then elevated to peak 3.45, CK normal, ESR normal, CBC normal, BMP normal. Chest x-ray normal. ECG normal sinus rhythm, diffuse ST changes and J-point elevation in inferolateral leads. Echo obtained read as normal, with EF 60% without pericardial effusion. Working Diagnosis pericarditis/myocarditis. Cardiology and infectious disease consulted, and started on ibuprofen, tylenol, IVIG, methylprednisolone. Labs for COVID antibodies, lyme, echovirus, HSV 6, EBV, CMV, Parvovirus B19, coxsackie, adenovirus pending. Cardiac MRI planned to be completed.

A 12-year-old male patient received bnt162b2 (PFIZER-BIONTECH COVID-19 VACCINE, Batch/lot number: unknown and Expire Date: unknown) on 22May2021 via unspecified route of administration as 1st dose, single dose for covid-19 immunisation. As medical history it was reported that, he had allergies and allergic to a lot of things, mold, feather dust and everything and dyspepsia. He had been taking Claritin for allergies (but not when he got the vaccine). She stated her son did not have any allergies to medications. Concomitant mediations were not reported. On 26May2021, 4 days after that he was complaining of chest pain. Caller wants to know if her son should get the 2nd dose of the vaccine. Caller stated, "He had other symptoms. On Tuesday the chest pain went away then complained again on Thursday. His doctor gave him an EKG and his heart rate was fine. Caller did not take him to the ER at the time because did not want a medical bill. His blood sugar spiked up. He had sweating, vomiting, diarrhea. He said he had "sharp pain in my chest and never felt that before" His blood sugar spiked up. He was dehydrated. He had ketones from it. Caller gave him extra insulin. He had chest pain from the vaccine. It was on Tuesday and happened again on Thursday but had not come back since then. It subsided. EKG was ok." No one was informed (doctors). I took him to the regular doctor. She did not know. She was not aware of the symptoms. They are not specialized in the medications they are giving. It was hard to give them any kind of referral. Both of us had vomiting, headaches and chills. He had never had chest pains from any vaccines. He had fatigue, these are symptoms form the covid shot. It was 4 days and then 6 days after the vaccine. A fast heartbeat. HCP are not going to help. If they are dosing the covid shots but they are not given the vaccines they did not want to do anything. There was no accountability. Caller having a hard time. No one can give me an answer, no one. Not from the experts, no one was telling me. The vaccine administration should have been through the doctors, they will not take your data without understanding the vaccines. All the kids 12 and up and all male and like my son and probably a lot more but they are not saying it. Caller scared. My son did not have a heart defect. Caller did not know. Heart inflammation; they need to know if it was safe or not. It was scary to know no one knows what was going on or not. Caller asked about myocarditis. The patient had heart burn before but said the chest pain was not that. With the symptoms the caller saw online, she thinks the patient had pericarditis. The patient's next dose was on 12Jun2021.The caller was reading online that people reported chest pain and ended up in the emergency room and the caller did not want that. The patient underwent lab tests and procedures which included blood glucose increased: spiked up on May2021, electrocardiogram: fine on 25May2021 EKG and his heart rate was fine, heart rate increased: fast on 25May2021. Prior Vaccinations (within 4 weeks) and AE(s) following prior vaccinations was none. Caller states the fever was more like 8 days and the diarrhea was at least 8 days, she would have to ask him when it completely went away. Caller states her son lost 5 pounds (on 20May2021) when he got sick on an unspecified date, he was 90 pounds currently, he was 95 when he got the vaccine. She states he kept losing weight over the week. Caller states his stool was green and he was pooping everything out, it was gross on an unspecified date. On the 2nd day, he experienced vomiting (on 21May2021). He was also tired for a long time on an unspecified date. Caller states her son had been staying at his dad's house when he was sick. She states she would've taken him to be seen earlier because after 5 days of fever, he needed an IGG drip in case it was his heart or similar to Kawasaki. She requested him to get an IgG Drip because, according to her, any child who has a fever for 7 days might have the virus that causes Kawasaki disease. This is based on her experience with her daughter who has Kawasaki disease. The son was not given the IgG drip because the doctors were not sure. They also tested his stool, but he did not have anything wrong. She mentioned that her son had COVID in October, the one that got sick for a week, then goes away, and comes back again. He was sick for a while with all the symptoms, and it was proven that he had it. Caller states when he got sick, they wanted to rule other things out, so they tested his feces for a bunch of things, but he was negative for everything. As of calling, the son had recovered but she was requesting for him to have an ultrasound of the heart. She was asking if he should get the 2nd dose. The events resulted in physician office visit. A 12-years-old male patient received bnt162b2 (PFIZER-BIONTECH COVID-19 VACCINE, formulation: Solution for injection, Lot Number: ER8729; Expiration Date: 31Jul2021) via an unspecified route of administration, in left arm on 27May2021 (at the age of 12-years-old) as dose 1, single for covid-19 immunisation. Medical history included patient had an allergic reaction to penicillin before. The patient's concomitant medications were not reported. The patient had a meningitis vaccine a month before the covid vaccine. Patient's mother was calling on behalf of her 12 year old son. She stated her son received the first dose of the Pfizer COVID vaccine on 27May2021. On 27May2021,6 hours after receiving the vaccine the patient said he was very itchy and had hives on the left side of his chest. At first she thought the patient had poison ivy because it was like 3-4 inches across. The next day the patient said he was itchy when he sat up and it went up onto his neck and cheeks and that's when she realized it was hives. She called the doctor, applied cold compresses to the patient and gave him Benadryl. The hives resolved very quickly, the patient has never had hives or an allergic reaction before. By 2:30 the patient had the Benadryl and you couldn't see the red anymore. The patient could sit up but kept saying it itched and that went on for about 48 hours. You would have to look pretty closely to see the bumps. One week after the patient received the vaccine he was really weak and tired (on 03Jun2021). She stated her son also keeps having episodes of tiredness and weakness. It felt like the patient had been exercising and his legs were tired and his head felt like it was burning (on 03Jun2021). The patient's temperature was 97.7. The patient laid down and he slept pretty hard, and was drooling, for a couple of hours. When the patient woke up he didn't know where he was, the time or date and within a couple of hours the patient was fine. She didn't think anything of it the first time it happened. This event happened again 2 weeks after the vaccine. The doctor said they symptoms sound like mono but the patient's lymph nodes aren't swollen and he doesn't have a sore throat. The doctor thought the patient was having a delayed immune response to the vaccine because of the timing of it. The doctor said that by 3 weeks this should have resolved. The patient has had 2 mild episodes of this since, but he didn't go to sleep he just laid down. The doctor said to wait to get the second dose, so now it is 4 weeks since the first vaccine. The patient was at the 30th percentile for his height and average for his weight. The patient had gained weight since the vaccine but his mother didn't know if that was related to the vaccine. She was afraid of the patient having myocarditis with the second dose. The patient went to sleep after taking the Benadryl last time and she was afraid that the patient won't be able to tell her if he was having chest pain or problems breathing. She stated if his reaction was just hives she would give him Benadryl but with the risk of myocarditis after being given Benadryl he wouldn't be alert enough to tell her if something was going on with his heart due to him having fatigue as well already. The redness went away within 24 hours and the itching was gone in 48-72 hours. The redness was gone the evening of the 28th. The patient could still see and feel the bumps, the bumps were just white. The patient's head was burning happened the first 2 times he had these events. The patient's temperature was 97.7 and 98.1 fahrenheit when this happened. The other two times the patient said his head burned between his eyes and not like a headache. The doctor said that if the fatigue symptom continued he would recommend doing some blood work but since it has gotten better his mother may take him to an allergist. She thinks that the patient may be allergic to something in the vaccine. she was thinking of taking him to an allergist to see if he was allergic to one of the ingredients in the vaccine. She stated from the list of the ingredients the only ingredient she found that could be linked to the vaccine that could cause an allergic reaction would be PEG. She stated her son never had any allergies to medications or vaccines in the past. She stated she was not sure what advice should be given regarding her son receiving the second dose of the vaccine due to him having an allergic reaction to the first dose and tiredness and weakness. She stated she was not sure of the risks of vaccine versus him not being vaccinated. She would like to know if he should receive the second dose after experiencing side effects with the first dose. Reporter seriousness events was unspecified. Outcome of the events hives and itching recovered on May 2021, redness recovered on 28May2021, event tiredness and weakness was recovering and for other events was unknown.

He received the second dose of the vaccine on 6/4. 6/6 he developed neck pain that developed into chest pain as well. He additionally had some shortness of breath. He was seen on 6/7 by his pediatrician. An electrocardiogram was obtained which showed ST elevation in the lateral precordial leads(image below). Additionally a chest x-ray was obtained demonstrating clear lungs and no notable findings. A troponin T was obtained and measured 232 ng/L(normal range 0-22). CRP 39 mg/L(normal 0-7). His chest pain resolved within 3 days of developing. He has not noted any palpitations. He has not had any syncope. He has been doing light activities without having any issues.

Stated that he had never been sick before. Stated that he was only 12 years old. Does not think that vaccine was adequate for children. At approximately 10:36 AM the client went limp, pale, and complained of vision changes to his guardian. The RN at observation went over to assess the patient with other onsite staff. Patient appeared pale and was alert and oriented x 3. Extremities appeared flaccid. The patient denied difficulty breathing, lungs were clear bilaterally. Client reported vision changes and stated "everything looks dark and blurry". Vital Signs were taken at approximately 10:37 am- Temperature was 99.1; Pulse 58; RR: 12; B/P: 104/59; O2: 99%. Second B/P was taken at 10:44 and was 111/63. Client denied eating breakfast or drinking water today. No medication was given. Denied having difficulty swallowing. Water was provided. EMS was called. EMS assessed client- B/P 98/66; Pulse 86; RR 14; O2 98% . At time of EMS assessment, the client reported feeling "normal" and denied having any signs or symptoms. After assessment, EMS reported that the client does not require an ambulance. Patients guardian, decided to opt out of ambulance transportation and opted to take the client to his health provider. The client and his guardian left at 11:01 am to be evaluated at pcp.

Three days after the vaccine, he started having chest pain and shortness of breath with exertion. He continued to be fatigued. Chest pain was intermittent. Saw pediatric cardiologist at outpatient appointment on 6/15/21 where he had a normal EKG and ECHO. High sensitivity troponin on 6/15/21 was 0.08. He had a Holter monitor placed. He was asked to do activity while wearing Holter monitor and self-limit based on symptoms. He played basketball that night and had to press the Holter button 8 times while playing when he was experiencing chest pain. When he got home from practice, he felt weak and dizzy with associated chest pain. Mom notes his nails were blue/purple, and he was "lethargic" . Symptoms improved with ibuprofen. Patient did not want to do usual activity, mom called cardiology who recommended going to ED. Patient has not had chest pain since coming home from practice on 6/15/21. He was admitted to hospital on 6/16/21. Repeat high sensitivity troponin was 0.82 on 6/16/21 and repeated on 6/17/21 was 0.41. Cardiology consulted. Cardiac MRI performed with normal results. He was dizzy on 6/17/21 when he got up to use the restroom, self-resolved. Cardiology evaluated him and cleared him for discharge with outpatient follow-up and self-limitation with activity.

He woke up with chest pain in the upper right quadrant at midnight and then it intensified when he was breathing deep breaths. They woke him up when he was sleeping and did not go away. Then, it continued through the next day and we asked our doctor if we needed to keep an eye on him because of all the reporting and stuff about the heart problems. They did a triage consult visit to see if it could be an adverse event from the vaccine. The course of action was to take 2 ibuprofen every 4 hours and use ice and heat to see if the pain improved to rule out musculoskeletal situations because that was what they thought it was. We did that for 2 days and then the pain moved to his back upper right quadrant and we noticed a rash all over his abdomen and we were not sure where he got that. Based on the fact that the pain moved the doctor was more confident that it was not an adverse event.

He woke up few minutes prior to the event and this event started after he passed urine. As soon as he finished Urination, he felt nauseated, dizzy and then lost consciousness thereafter. He fell backwards as a result and hurt his back as well as back side of the head. During his unconsciousness his body was completely stiff and his eyes were rolled back. For few seconds then he became completely still (it felt he was not breathing). I gave him physical stimulus (slight slaps on the face) post which he started reviving. During this time we called 911. I gave him few sips of water forcibly and he started reviving thereafter. on 5/15 and 5/16 (post vaccination on 5/14) he was feeling tired and seemed bit dull. This was unusual as Pt is a competitive soccer player and highly active.

After a little while, he got up and was able to take some Tylenol and attend his video school day with frequent breaks. In afternoon, he started to feel worse. 102. 6 fever - we called Tele-doc appt. His throat was sore and he had some chest pain. The doctor recommended that we go to ER to have him fully evaluated. Pain and fever got better with the Tylenol. He took another couple of dose. Next morning, he had a lot better but continued to have a low fever 100.2 - on and off that day. His lymph nodes under his left arm were quite swollen and painful. The fever did not continue yesterday but the swelling has and the pain has. He was very light-headed and brain fogged on Sunday and Monday. Seems to be better today - the lymph nodes have gone down a lot but are still swollen today. He is in process of recovering.

Spoke with Mom on 6/1/21 is improved History of Present Illness HPI Narrative: 14-year-old boy with history of autism presents with behavior changes, nausea, vomiting. Patient and his mother provide the history. He was in his usual state of health last night, had a normal dinner. Did not answer his door this morning. Parents found him staring off into space with decreased level of responsiveness. He was able to drink fluids. Patient reported having a few episodes of vomiting and abdominal pain earlier. They were concerned that he could be having a reaction to his COVID-19 vaccine which was given 4 days ago. Mom gave 25 mg of Benadryl at home. They were driving to Hospital for evaluation when the patient started looking worse. He has been hyperventilating but also has now clenching of hands and reports that his feet feel funny. Mom reports that he is not at his behavioral baseline currently. No prior abdominal surgeries or additional issues. Not currently take any medications. Patient denies taking any Illicit medications/substances.

Worse with exertion, and also worse with leaning forward and also lying flat. Went to PCP who drew troponin, which was elevated, sent the pt to the ER. Troponin was elevated in the ER, ECG was normal, echo also normal. Admitted for trending of labs, work up for infectious causes of myocarditis sent. cMRI showed LGE in small area of the inferolateral wall of the LV. Symptoms resolved with Tylenol prior to arrival at the ER, discharged after ~24 hours observation on ibuprofen RTC with close follow-up planned. Restricted from strenuous activity.

This patient received their second Pfizer COVID-19 vaccine on 6/4/2021. The patient began experiencing arm pain which migrated to his chest on 6/6/2021. The pain resolved but returned with increased severity on 6/7/2021, prompting medical evaluation. The pain was described as a pressure sensation in the center of the chest and in bilateral arms. Upon initial evaluation, he was found to have an abnormal EKG with ST segment changes. Troponin was significantly elevated. He was transferred to the hospital. A transthoracic echo was obtained and was normal. He was hospitalized on the pediatric cardiology service, admitted on 6/8/2021. Troponin trended down during his stay. He was started on lisinopril. He was discharged home on 6/9/2021 with plan for outpatient pediatric cardiology follow up. Given the proximity of his second Pfizer COVID-19 vaccine to onset of his symptoms, and given similar reported events, the chief concern is whether his episode of myocarditis is related to administration of the Pfizer COVID-19 vaccine. I was on call as attending physician when this patient was admitted. Physician cared for this patient as the pediatric cardiology inpatient attending during his hospital course.

Received 2nd COVID-19 vaccine on 6/14 and developed headache and myalgia that evening. On 6/15, continued to have these symptoms then developed abdominal pain and vomited once. Symptoms persisted on 6/16; then developed chest pain at 11:00PM and reported that pain seemed worse when inhaling and was sharp, shooting pain. Presented to ED at 12:00 AM on 6/17 and subsequently transferred to Pediatric ICU due to elevated troponin and d-dimer. Initial EKG showed tachycardia. Cardiologist consulted. He had an echocardiogram on 6/17 AM that showed mildly depressed LV systolic function with EF ~58%. Troponin was elevated from 2.9 to 16.9. Due to echo and elevation in troponin patient received 100 gm IVIG (max dose based on IBW). Has required one dose of 15 mg ketorolac IV and 650 mg acetaminophen PO x1 for chest pain. At 1:00 PM on 6/17, troponin increased to 54.6 and aspirin 81 mg started. Chest x-ray showed borderline cardiomegaly. On 6/18 repeat echocardiogram with normalized LVEF and considered a normal echocardiogram per cardiologist.

On the second day of his hospitalization he was no longer ambulatory. Diagnosed with likely AIDP based on CSF studies (albuminocytologic dissociation, negative infectious work-up). Treated with 3-day course of IVIG (7/21-7/23), after which he showed significant improvement in his upper/lower extremity and facial strength and ambulation. He is being discharged with ongoing outpatient physical therapy.

Mom states that eyes were darting back and forth at this time. He was not responding to her shouting his name and his left arm was curled in towards his body with the rest of his body remaining stiff. This lasted approximately 1 minute, followed by a time of confusion and drowsiness. Approximately 1 minute later mom states that patient's eyes rolled back to his head and he had full body movement consistent with tonic-clonic seizures. Family were able to lay him onto the ground during this time. Mom denies any color change, apnea, or loss of bowel or bladder. States he did have one episode of emesis following the event, and 2 episodes of emesis in the ambulance on the way to ED. Patient has no previous history of seizures or seizure-like activity. No new ingestions. Has not been sick recently and denies any new sick contacts. There has been no head trauma. Per Mom, patient's dad has a history of "partial seizures" but never required any medication for them. At arrival to the ED he continued to be drowsy and not at his normal baseline. Continue to hold his left wrist up toward his body in a contracted manner and tended to have repetitive speech. Briefly, ER course significant for a CT head and evaluation by pediatric neurology. CT head was unremarkable for acute intracranial process. A CBC, CMP, and coags were also obtained due to patient's continued contracture positioning and the possibility of a stroke. CMP was unremarkable with a sodium of 139, potassium of 3.7, chloride of 107, bicarb 25, BUN of 12 and creatinine of 0.64. CBC was also unremarkable with WBC of 10.72, hemoglobin of 14.8, hematocrit of 44.4 and platelets of 370. There was 81.77% neutrophils after evaluation by neurology it was felt that the patient remained postictal and recommended that he be admitted on continuous EEG and possible MRI in the a.m.

In the morning pain much improved, but still with pain on deep inspiration relieved with leaning forward. No other vaccines given in the past year. Since the vaccine, pt has had fever up to 102. Continued headache, neck stiffness. 1st night 6/3/21- bad headache, throbbing headache all around head but worse back right side and then "spikes" on left side (like someone was hitting him with an axe) and fell asleep d/t sever pain, trouble breathing in the next few days 2nd night 6/4/21- neck pain and stiffness was really bad, he looked pale and almost disoriented. Had fever. Slow to respond. Would get rush of bad headache if he moved his neck too fast. Afraid he would fall down if he moved too fast. 3rd night 6/5/21 - (called 911) chest pain, difficulty breathing, vomited stomach acid, pain radiated down chest to the left, pain only when breathing in and went away the next AM 6/7/21 Chest still feels sore on Left side. A 15-year-old male patient received second dose of BNT162B2 (Pfizer Covid-19 vaccine; Solution for injection; Batch/Lot Number: EW0185) intramuscular, administered in Arm Right on 04Jun2021 as 2ND DOSE, SINGLE DOSE for covid-19 immunisation. The medical history and concomitant medications were not reported. The patient previously took first dose of BNT162B2 (Batch/Lot Number: EW0182) on right upper arm on 14May2021 for covid-19 immunisation. The patient reported, had his 2nd vaccine on Friday 2 days ago yesterday he just had congestion, today on 06Jun2021, he woke up with chest pain and he said his upper back was aching and he has chest pain. He is also having temperature of 100 degree, so Tylenol was given. Sinus congestion and pain was yesterday on 05Jun2021. He does not have congestion today. Yesterday for sinus pain and congestion, the patient was given Sudafed 1 tablet. It is an over-the-counter medicine, 30 milligrams 1 tablet. A history of sinus was reported for about everybody in his family got sinus one time in life. The outcome for the events sinus congestion and pain was recovered on 06Jun2021. Outcome for the events chest pain, upper back was aching and having temperature of 100 degree was unknown.

Patient received his 2nd Pfizer COVID vaccine at 1650 on 6/10/21. Just over 24 hours later he developed fatigue and fever to 102.1. Mom gave him ibuprofen around 1830 on 6/11/21. His temperature was <100 within 45 minutes of the ibuprofen. He slept the remainder of the night, but was diaphoretic most of the night. Approximately 0915 on 6/12/21 Patient developed a severe pain in his mid-back. Due to his known risk of aortic dissection the family took him immediately to the ED. A CTA was rapidly obtained. He was noted to have a proximal abdominal aortic aneurysm and dissection and was transferred to Hospital y air. Upon review of the images with the surgical team he was then transferred to Hospital and Clinic for surgery, which was performed at 0730 on 6/13/21. At the time of this reporting he is presently in the operating room, so the outcome of surgery is yet unknown.

Patient was also perspiring all over the body, (arms, face, etc) Patient was also warm and clammy. Patient was disoriented and unable to respond typically. 911 was called. Patient was given a sprite and could not hold the sprite independently. Directly after, blood pressure Pulse and oxygen levels were checked and were below normal. (Blood pressure - 54/47) (Pulse - 87 BPM). Patient then became more responsive and was able to say name and specified location upon asking. Within 5 minutes of 911 call, ambulance arrived and emergency response took over care. Patient was then transported to local hospital.

My son developed neck soreness and and an enlarged lymph node that weekend. On Monday following the vaccination a severe headache started which lasted 4 days, even with tylenol and ibuprofen. On Tuesday, he started complaining about not remembering what he did the day before when he woke up in the morning..memory started coming back around noon. The memory issues lasted one week and began to dissipate thankfully. After a few weeks got a call to schedule his second shot. Called the pediatrician and the pediatrician told us not to administer the second vaccine due to adverse reaction to the first. We are monitoring his memory issues..he is almost a straight A student in school last year and on an Advanced Learning Plan in high school due to high IQ. We will know if this is long lasting after attending school and monitoring his grades and headaches in the fall.

Symptoms improved Monday morning with resolution of back/neck and arm pain but continued to have constant chest pain/pressure. Seen by PCP on 6/14 where labs were drawn which resulted on 6/15 and revealed elevated troponin. Patient directed to ED where EKG, labs concerning for myocarditis then transferred to hospital ED. He continues to be hospitalized on the floor at hospital (no ICU stay required) now with improving cardiac markers.

The rashes looks like big blotches/ map looking red marks all over his whole body. His face was all covered with the blotches of rashes including his scalp. He had no fever though. He was breathing normally but the rashes were very itchy. gave him double dose of Benadryl. Then switched to adult Benadryl but still taking double the dose. This went on for 2 days and no sign of improvement despite taking Benadryl every 6 hours. Patient was sleeping most of the days due to the medication. 6/15/2021: reached to Family doctor and explained the situation. doctor recommended to go to ER after seeing pictures of the rashes. Patient adamantly refuses to go. Doctor suggested take Allegra on top of Benadryl. Patient took Allegra 24D. Patient mostly sleeping. 6/16/2021: saw some slight improvements. Back and chest cleared of the rashes but still heavy on the arms, most especially the thigh area and legs. still taking Benadryl but only the normal dosage. 6/17/2021: All rashes are gone and cleared/

Starting from the night of the second day (i.e., June 10th), the patient feels chest pain. The pain also extends to neck and left shoulder. Deep breath aggravates the pain. The patient could not fall asleep. He had a light fever at the night of vaccination day but the fever was gone before the chest pain. Single lead ECG with Watch has been measured and sent to a friend of the parents' who is a physian, and no abnormal sign was shown from ECG. 400 mg Advil (ibuprofen) was given in the coming morning around 7 AM and the patient was able to fall asleep and symptons were gradually relieved. Another two doses of 200 mg Advil were given with six hours apart on the same day. The pain was gone by the following morning (i.e., June 11th) and no ibuprofen has been given afterwards. We suspect that this is related to myocarditis or pericarditis. This was the second dose of Pfizer COVID-19 vaccine. There was no symptons for adverse effect after the first dose.

15-year-old male previously healthy who presented to the ED on 6/23/2021 after he presented with chest pain starting the night of 6/22/2021. He received his second dose of Pfizer Covid vaccine on 6/19/2021 (1st one was 5/29/21, at pharmacy). Same day he received the vaccine he started w fever as high as 39.5, headache and myalgias for which mom gave him iburpofen. Symptoms improved after 2 days but he continued with headaches and the night of 6/22/21 he woke up with chest pain. Did not take anything for the pain and he woke up the next day with persistent pain (midretrosternal) so mom took him to ED but it was recommended he be evaluated here. In the ER he reported 4 out of 10 chest pain and mild right temporal pain. No nausea, no abdominal pain. No shortness of breath. . In the ED vital signs were normal with a heart rate of 61 and normal blood pressure. O2 sat was 98% in room air. A troponin was done that was elevated at 1.96. Chest x-ray was normal. EKG showed sinus bradycardia otherwise normal EKG. he was given 600 mg of ibuprofen in the ED and he was evaluated by cardiology and since his symptoms were stable and patient was well-appearing and improved he was sent home on ibuprofen as needed. Mom reports that by the next day he was back to normal and has not complained of pain, palpitations or shortness of breath or any other symptoms since then. Denies feeling tired. He has not allow him to have any sort of activity. He was seen by cardiology yesterday and a EKG and echo were done that were unremarkable and he was placed on a continuous monitor for arrhythmias the next month.

15 yo M with no PMH p/w 1 day of chest pain and elevated troponin and lateral ST segment elevations. On Wednesday 6/30 he had his second dose of Pfizer COVID vaccine. There after he had headache, tactile fever, fatigue and myalgias which resolved by Friday 7/2. On Friday at midnight, he awoke with 7/10 sharp substernal chest pain, non radiating. No recent trauma to the chest. He took Tylenol at home which did not relieve the pain. No recent URI sx over the last month. The last time he remembers having URI sx was 1.5 years ago. He does endorse "one or two sneezes" over the past month, which he attributes to allergies. Today 7/3, he presented to urgent care center where he got an EKG, which was concerning for ST elevations. He was given ASA which did not relieve the pain. He was sent to hospital for further work up where he had a Troponin to 1.59, CRP to 36, AST to 79, ALT to 64, Alkphos to 74, BNP to 119, ESR to 10, and WBC to 13.4. EKG showed ST elevation in I, II, precordial leads V4 V5 V6. CXR wnl. He was given IV toradol at hospital, which alleviated his CP. He was transferred to ED for further evaluation. On arrival to the ED on 7/3, he was HDS afebrile, with BP 111/69, HR of 80, and satting 100% on RA. His CP had resolved to a level of 0/10. Additional labs were obtained including repeat CBC (with WBC 10K, Hgb of 14), normal ESR of 20, normal D-dimer of less than 0.27, BMP wnl, elevated LFTs with AST 103, ALT of 61, elevated LDH to 293, normal GGT and alk phos, normal BNP to 28, elevated CRP to 3, and an extremely elevated troponin of 3.15. NP swab for COID, Influenza A/B, and RSV were negative. CXR wnl but did note findings consistent with degenerative disc disease at one level in the mid-distal thoracic spine. Hospital Course: Patient was admitted to the cardiology floor for continued monitoring of his troponin levels and EKG. He was then treated with IVIG 2g/kg and started on methylprednisolone IV x 8 doses. His chest discomfort improved by day 2 of admission. An echocardiogram was performed which did not show any cardiac dysfunction. His troponin downtrended while on IV steroids, therefore he was transitioned over to PO steroids which he tolerated well. On day of discharge, his troponin levels were continuing to trend down, and EKG was WNL. Cardiac MRI was performed and showed late gadolinium enhancement in the left ventricle. He had occasional NSVTs/PVCs; he was started on Bisoprolol 2.5mg daily to prevent arrhythmias after discharge. He was discharged home with a Holter monitor to be worn for 4 days. Viral myocarditis studies were sent and all negative. By time of discharge, patient was well-appearing, vitals stable, demonstrating good PO intake. Prescriptions sent to preferred pharmacy. Follow up with cardiology in place. Discharge instructions and return precautions reviewed with patient and parent, who expressed good understanding and agreement with plan. Patient will follow up in cardiology clinic next week with a repeat MRI 3-6 months. Of note, CXR from 7/3 with incidental findings suspicious for degenerative disc disease at one level in the mid-distal thoracic spine. This should be followed by his primary care physician as an out patient. Reasons for new, changed, and discontinued medications: - Bisoprolol 2.5mg daily (for prevention of NSVT) - Prednisone 30mg BID (myocarditis) - Famotidine 20mg daily (while on steroids) Reasons for new, changed, and discontinued equipment: NA Relevant Diagnostic Images/Studies: Cardiac MRI (7/7): ? Normal biventricular size and systolic function. ? No regional wall motion abnormalities. ? Suggestion of increased T2 signal intensity/edema. ? Positive myocardial late gadolinium enhancement without functional correlate. ? No significant valvular dysfunction. ? No coronary artery aneurysms. ? Small pericardial effusion. Echocardiogram (7/3): ? Normal valvular function. ? Normal left ventricular size and systolic function. ? Normal diastolic function indices. ? Reduced longitudinal strain with normal circumferential strain. ? Normal appearing proximal coronary arteries. ? Qualitatively normal right ventricular systolic function. ? No pericardial effusion present. ? A comprehensive anatomic survey was not performed at this time. The child's arms and legs were stiff and stretched out and per father's observation, he claimed that his child's eyes were rolled backward. The child returned to "normal" status, he claims that he doesn't remember what happened. The child's hands were little shaky as he was attempting to drink water. We asked multiple questions - child claimed he was not nervous about the vaccine. Parents claimed that the child is doing better with vaccine anxiety compare to previous time, The child and parents informed that he did not eat anything yet. The child claimed that he is not dehydrated. 911 was called immediately at sight. EMT took the child's vital and reevaluated the child's condition. family wanted to follow up with their doctors (if correct, I believe EMT's vital check indicated 116/58 for BP) - our temperature measurement showed 97.3.

Lot Number: EW0202), dose 1 via an unspecified route of administration, administered in Arm Left on 05Jun2021 (at the age of 15-years-old) as dose 1, single for COVID-19 immunization. No medical history was reported. Patient had no known allergy. Other medical history was none. No concomitant medication was reported. Patient did not receive any other vaccines within 4 weeks prior to the COVID-19 vaccine. Patient did not receive any other medication within 2 weeks prior to the COVID vaccine. The patient had not been tested for COVID-19 nor before neither since the vaccination. Facility where the most recent COVID-19 vaccine was administered at Pharmacy or Drug Store. The patient experienced pupils dilated and could not see, shaky, rigid, and was very hot to touch on 05Jun2021. This was within a few minutes of receiving the vaccine and lasted about 20-30 minutes. Seriousness criteria was reported as no. As AE treatment, Pharmacist used cold compresses and gave him juice. The outcome of the events were recovered on 2021. No follow-up attempts are possible. No further information is expected.

Labwork done at that time showed modest nonspecific elevations of inflammatory markers CRP of 42, (normal < 8), ESR 50, calprotectin 285 (120 limit). SARS covid-19 antibody semiquant to spike protein RBD was 64.4 U/mL (<0.8, Labcorp). Other tests did not identify another specific etiology, and an evaluation by another physician for myocarditis was reportedly normal. As of 7/17 he feels better with a resolution of all symptoms, labwork repeated this past week (7/15) showed a resolution of those same inflammatory markers to normal levels .

6/26/21--;pca; 7/8/21 mild, mildly pruritic , non-painful, bilateral conjunctival injection (left more than right) without ocular discharge--attributed to seasonal allergic conjunctivitis butstill persists at present time. 7/13/21 woke up with severe (8-9/10) sub sternal and left sided chest pain/pressure sensation, whih radiated to his back with associated difficulty breathing/shortness of breath, worsening of pain with deep inspiration and sling supine, and palpitations. Bilateral calf pain and tenderness fo left foot. Symptoms persisted. To ED at Hosptial but left without being seen due to long lines then came to our ED for evaluation. Of note received 1st dose humira without apparent side effects

7/9/21 developed moderately severe throbbing headache localized to his forehead with moderately sever painful "tight" sensation in the muscles of the back of his neck and lower back. Pain persisted for ~ 3 hours ; Temp 101 Tylenol l taken but fever and pain returned after 3-4 hours. 7/10 awakened with nausea, dizzy/lightheaded , headache, vomiting. symptoms continued until 7/12/21. Viral infection suspected but nonetheless prescribed cephalexin "in case". Fever and headache continued with once daily eesis so came to ED for evaluation 7/14/21

Per MD Note: "Acute throat and upper chest pain this AM. At approximatley 0300, he woke up with severe 10/10 chest pain. The pain is sharp and he points to the midline between his collarbones. He went to an ED and received a GI cocktail and famotidine, which did not provide much benefit. He had an elevated troponin to 2.13, EKG without evidence of pericarditis. He was given 0.5 mg Ativan and 15 mg IV Toradol. Workup notable for CXR (wnl), EKG (wnl), CRP (2.2), ESR (29), proBNP (62 wnl). Lipase (wnl), Lithium 0.8 (0.6-1.2 mmol/L). CMP 142/3.9/106/27. BUN 12, Cr 0.75. Dexi 120. Upon arrival to our ED, he reports 1/10 chest pain. He denies any SOB, difficulty breathing, palpations, abdominal pain, nausea, vomiting, or diarrhea. He received his second Pfizer COVID-19 vaccination on 7/19 and has had low grade fever (Tmax 100.5F), myalgias, tiredness, and headache since then. He reports that his chest feels slightly tight, but denies any SOB. Assessment: Patient is a 15 y.o. male presenting with acute onset of chest pain and elevated troponin. Workup has been extensive and including BNP, repeat troponin, EKG, CXR, and echocardiogram. He does not show evidence of severe myocarditis or pericarditis. Assessment by cardiology suggested that mild myocarditdis in the setting of recent COVID-19 vaccination and acute heart strain/troponin leak in the setting of acute pain"

I don't know if any of his adverse events are related I wanted the CDC to be aware. He started feeling uneasy, unwell, vomiting, itchy ears (both), upper chest pain, upper back pain and difficulty breathing. We went to the ER at 17:00 - 22:00 on 7/18 was discharged couldn't figure out what was happening was given medication for migraine, Benadryl, Loratadine.

He was having severe chest pain 10/10; Shortness of breath; Neck pain that radiated through his spine; This is a spontaneous report from a contactable physician (patients mother). A 15-years-old male patient received bnt162b2 (BNT162B2, formulation: Solution for injection), dose 1 via an unspecified route of administration on 15May2021 (Batch/Lot number was not reported) as 1ST DOSE, SINGLE for covid-19 immunization. The patient medical history was not reported. The patient's concomitant medications were not reported. On 16May2021, the next night (second day) he was having severe chest pain 10/10, shortness of breath, neck pain that radiated through his spine, pulse oxygen level 98 percent and his heart rate was fine. Patient received the first dose of the covid vaccine on Saturday, and Sunday is when the severe symptoms started. Caller stated she gave him tylenol and then he was fine. On the third day symptoms improved. By Monday, patient was better. Caller stated her sons next dose is scheduled 05Jun2021. Reporter read new reports about myocarditis. Reporter does not know who to contact to figure out if patient should get the second dose of the covid vaccine. Reporter heard that after the second dose of the covid vaccine, symptoms get worse. Caller stated she had reported on V-safety. Reporter declined to complete a safety report. Reporter has already reported through VAERS. The outcome of events was recovered. Unable to clarify details provided or gather additional information. Caller notified that medical advice is not able to be provided but may have resources to help answer questions. Caller was provided with phone number of (number withheld), option 3, and hours of operation of Monday through Friday, 8AM to 8PM, Saturday and Sunday 9AM to 3PM for COVID-19 vaccine inquiries, prior to attempted warm transfer. Unable to warm transfer due to high call volume. Caller agreed to cold transfer. Information on the lot/batch number has been requested.
